Messung von H2O2 im Atemkondensat bei Patienten mit allergischem Asthma mit und ohne Supplementation von mehrfach ungesättigten Fettsäuren

  • Es ist die Hypothese der hier vorgelegten Arbeit, dass bei Asthmatikern oxidativer Stress in der Lunge durch die nichtinvasive Methode der Atemkondensatsammlung und Messung der [H2O2] im EBC erfasst werden kann. Basierend auf dieser Hypothese sollte geprüft werden, inwieweit bei stabilen Patienten mit intermittierendem allergischem Asthma (Grad I, WHO) eine erhöhte [H2O2] im EBC nachweisbar ist und ob es zu einem differenten Anstieg der [H2O2] nach niedrig dosierter Allergenbelastung mit und ohne Supplementation von n-3 PUFA kommt. In dieser Arbeit wurden Patienten mit intermittierendem Asthma und Hausstaubmilbenallergie einbezogen und repräsentierten die Milbenallergikergruppe. Sie wurde in einer randomisierten, doppelblinden und placebokontrollierten fünfwöchigen Studie getestet. Die Patienten erhielten entweder in Gruppe A n-3 PUFA-reiche Kapseln oder in Gruppe B PlaceboKapseln über fünf Wochen. Nach einer dreiwöchigen Aufsättigungsphase wurden die Probanden über zehn Werktage zusätzlich mit niedrig dosiertem Milbenallergen provoziert (FEV1-Abfall 5 %). Atemkondensatuntersuchungen wurden mit Hilfe des ECoScreen® und des ECoCheck® (VIASYS Healthcare GmbH) vor Beginn (V1), nach drei Wochen (V2) und nach der letzten Milbenprovokation (V3) durchgeführt. Zur Erfassung der pulmonalen Entzündung erfasste man zusätzlich bei den genannten Visiten die [eNO] als Kontrollparameter. Weiterhin, zur Erfassung des normalen Verlaufs der [H2O2], wurde bei einer gesunden Kontrollgruppe, die weder provoziert noch supplementiert wurde, die [H2O2] im EBC nach identischem zeitlichen Schema erfasst. In der Milbenallergikergruppe nahmen 30 Patienten und in der Kontrollgruppe 13 Probanden an der Studie teil. Aufgrund technischer Schwierigkeiten und nicht auswertbarer Messwerte, wurden in der Milbenallergikergruppe 17 Patienten und in der Kontrollgruppe neun Probanden zu statistischen Analysen herangezogen. In der Verumgruppe (p = 0,246), der Placebogruppe (p = 0,180) und der Kontrollgruppe (p = 0,185) war kein signifikanter Unterschied hinsichtlich der [H2O2] im EBC zwischen den Visiten vorhanden. Zwischen der Verum- und der Placebogruppe bestand zu keinem Zeitpunkt ein signifikanter Unterschied. In der Milbenallergikergruppe unterschied sich der Verlauf der [H2O2] im EBC deutlich vom Verlauf der [eNO]: Die [H2O2] konnte allenfalls einen Trend (p = 0,101) zwischen den Visiten aufweisen, wobei die [eNO] einen hochsignifikanten (p < 0,001) Unterschied zwischen V3 und V2 aufwies. Es bleibt zum einen offen, ob n-3 PUFA einen antientzündlichen Effekt bei Asthma bronchiale (Grad I, WHO) hat, und zum anderen, inwieweit die Messung der [H2O2] im EBC anhand des ECoCheck® (Fa. VIASYS Healthcare GmbH) als diagnostischer Marker oder zur Verlaufskontrolle bei Asthma bronchiale (Grad I, WHO) geeignet ist.
  • It is the hypothesis of this study that oxidative stress in the lungs of patients with asthma can be recorded in the Exhaled Breath Condensate (EBC) using a non-invasive method of breath condensate collection and the measurement of H2O2 concentration ([H2O2]). Based on this hypothesis, we had to determine whether an increased [H2O2] could be detected in the EBC of stable patients with intermittent allergic asthma (grade I, WHO) and whether this increase varies between patients with and without supplementation of n-3 PUFA. Patients with intermittent asthma (grade I, WHO) and house dust-mite allergy were included in this study and represented the allergic patient group. They were tested in a randomized, double-blind and placebo-controlled five-week study. Patients received either n-3 PUFA-rich capsules in Group A or placebo capsules in Group B, over five weeks. After a three-week accumulation phase the subjects were given additional low-dose mite allergen provocation (FEV1-drop 5 %) over ten working days. Breath condensate studies were conducted using the ECoScreen® and ECoCheck® (VIASYS Healthcare) before the start (visit 1), after three weeks (visit 2) and following the last mite provocation (visit 3). The concentration of exhaled NO ([eNO]) data on pulmonary inflammation was also recorded during those visits as a control parameter. Furthermore, in order to determine the normal levels of [H2O2], we measured, following an identical time frame, the [H2O2] in the EBC of a healthy control group, which was neither provoked nor supplemented. Thirty patients in the mite-allergic patient group and 13 subjects in the control group took part in the study. Due to technical difficulties and measurements which could not be used for evaluation purposes, 17 patients in the allergic patient group and nine subjects in the control group were used for statistical analysis. In the active treatment group (p = 0.246), the placebo group (p = 0.180) and control group (p = 0.185), no significant difference in terms of [H2O2] in the EBC between visits occurred. Between the active treatment and placebo groups, there was no significant difference at any stage of the experiment. In the allergic patient group, the variations [H2O2] in the EBC differed significantly from the variations in the levels of [eNO]: the [H2O2] could possibly indicate a trend (p = 0.101) between visits, whereas the [eNO] showed a highly significant (p < 0.001) difference between V3 and V2. Two avenues of enquiry remain open: first, whether n-3 PUFA has an anti-inflammatory effect in bronchial asthma (Grade I, WHO), and second, how far the measure of [H2O2] in the EBC based on ECoCheck® (VIASYS Healthcare) is suitable as a diagnostic marker or follow-up control for bronchial asthma (Grade I, WHO).
