Background: Depression is a disorder with high prevalence in primary health care and a significant burden of illness. The delivery of health care for depression, as well as other chronic illnesses, has been criticized for several reasons and new strategies to address the needs of these illnesses have been advocated. Case management is a patient-centered approach which has shown efficacy in the treatment of depression in highly organized Health Maintenance Organization (HMO) settings and which might also be effective in other, less structured settings. Methods/Design: PRoMPT (PRimary care Monitoring for depressive Patients Trial) is a cluster randomised controlled trial with General Practice (GP) as the unit of randomisation. The aim of the study is to evaluate a GP applied case-management for patients with major depressive disorder. 70 GPs were randomised either to intervention group or to control group with the control group delivering usual care. Each GP will include 10 patients suffering from major depressive disorder according to the DSM-IV criteria. The intervention group will receive treatment based on standardized guidelines and monthly telephone monitoring from a trained practice nurse. The nurse investigates the patient's status concerning the MDD criteria, his adherence to GPs prescriptions, possible side effects of medication, and treatment goal attainment. The control group receives usual care – including recommended guidelines. Main outcome measure is the cumulative score of the section depressive disorders (PHQ-9) from the German version of the Prime MD Patient Health Questionnaire (PHQ-D). Secondary outcome measures are the Beck-Depression-Inventory, self-reported adherence (adapted from Moriskey) and the SF-36. In addition, data are collected about patients' satisfaction (EUROPEP-tool), medication, health care utilization, comorbidity, suicide attempts and days out of work. The study comprises three assessment times: baseline (T0) , follow-up after 6 months (T1) and follow-up after 12 months (T2). Discussion: Depression is now recognized as a disorder with a high prevalence in primary care but with insufficient treatment response. Case management seems to be a promising intervention which has the potential to bridge the gap of the usually time-limited and fragmented provision of care. Case management has been proven to be effective in several studies but its application in the private general medical practice setting remains unclear.
The focus of the discussion at the conference on September 23, 2004 was on the long-term impact on capital markets and pension systems. The speakers tried to identify the direction and magnitude of potential changes as well as the likelihood of an eventual asset meltdown. The conference's objective was to combine insights from academia with those from the financial community in order to provide a more comprehensive outlook on capital market developments. Der italienische Büchertisch ist reich gedeckt. Enger wohl als anderswo ist die Verbindung zwischen der Welt der Gelehrten, der Universitätsausbildung und dem breiten Publikum. Es ist nicht anstößig, ein großes Thema auf hundert Seiten gemeinverständlich zu behandeln, nur die wichtigste Literatur zu verzeichnen und das Ganze in gefälliger Form unter die Leute zu bringen. Der Verlag Laterza hat mit seiner kleinformatigen Serie "Universale" gerade die Nr. 856 erreicht, in einer parallelen "Biblioteca Essenziale" die Nr. 56. Dort gibt es Klassikertexte, "Einführungen" in alle Künste und Wissenschaften, aber auch zugespitzte Thesen, aus der Rechtsgeschichte etwa von Bretone und Talamanca "Il diritto in Grecia e a Roma" oder Grossis "Prima lezione di diritto". Auch die beiden letzten Bändchen beider Reihen, die hier vorgestellt werden, stammen von bekannten Rechtshistorikern. ...

In recent years stock exchanges have been increasingly diversifying their operations into related business areas such as derivatives trading, post-trading services and software sales. This trend can be observed most notably among profit-oriented trading venues. While the pursuit for diversification is likely to be driven by the attractiveness of these investment opportunities, it is yet an open question whether certain integration activities are also efficient, both from a social welfare and from the exchanges' perspective. Academic contributions so far analyzed different business models primarily from the social welfare perspective, whereas there is only little literature considering their impact on the exchange itself. By employing a panel data set of 28 stock exchanges for the years 1999-2003 we seek to shed light on this topic by comparing the factor productivity of exchanges with different business models. Our findings suggest three conclusions: (1) Integration activity comes at the cost of increased operational complexity which in some cases outweigh the potential synergies between related activities and therefore leads to technical inefficiencies and lower productivity growth. (2) We find no evidence that vertical integration is more efficient and productive than other business models. This finding could contribute to the ongoing discussion about the merits of vertical integration from a social welfare perspective. (3) The existence of a strong in-house IT-competence seems to be beneficial to overcome.
Erst die Umsetzung der zahlreichen bestehenden Planungen und Ideen kann positive Folgen für den Naturschutz bedrohter Arten und deren Lebensräume nach sich ziehen. Die Durchführung solcher Maßnahmen kostet aber viel Geld, das den Akteuren häufig fehlt. Trotz geschmälerter öffentlicher Haushalte ist die Chance für das Auffinden von Geldquellen aber gar nicht so schlecht, wie das „Finanzierungshandbuch für Naturschutzmaßnahmen“ des Bundesministeriums für Umwelt, Naturschutz und Reaktorsicherheit erkennen lässt.
