Stammzellen aus dem Knochenmark werden seit Jahrzehnten gegen Blutkrebs eingesetzt. In der Zukunft sollen auch andere Krankheiten mit Stammzellen und therapeutischen Genen behandelt werden. Die an der Goethe-Universität geleisteten Vorarbeiten zeigen, dass der Standort wie kaum ein anderer geeignet ist, diese neuen und maßgeschneiderten Verfahren voran zubringen.
Background: Chemotherapy of glioblastoma is largely ineffective as the blood-brain barrier (BBB) prevents entry of most anticancer agents into the brain. For an efficient treatment of glioblastomas it is necessary to deliver anti-cancer drugs across the intact BBB. Poly(lactic-co-glycolic acid) (PLGA) nanoparticles coated with poloxamer 188 hold great promise as drug carriers for brain delivery after their intravenous injection. In the present study the anti-tumour efficacy of the surfactant-coated doxorubicin-loaded PLGA nanoparticles against rat glioblastoma 101/8 was investigated using histological and immunohistochemical methods. Methodology: The particles were prepared by a high-pressure solvent evaporation technique using 1% polyvinylalcohol (PLGA/PVA) or human serum albumin (PLGA/HSA) as stabilizers. Additionally, lecithin-containing PLGA/HSA particles (Dox-Lecithin-PLGA/HSA) were prepared. For evaluation of the antitumour efficacy the glioblastoma-bearing rats were treated intravenously with the doxorubicin-loaded nanoparticles coated with poloxamer 188 using the following treatment regimen: 3×2.5 mg/kg on day 2, 5 and 8 after tumour implantation; doxorubicin and poloxamer 188 solutions were used as controls. On day 18, the rats were sacrificed and the antitumour effect was determined by measurement of tumour size, necrotic areas, proliferation index, and expression of GFAP and VEGF as well as Isolectin B4, a marker for the vessel density. Conclusion: The results reveal a considerable anti-tumour effect of the doxorubicin-loaded nanoparticles. The overall best results were observed for Dox-Lecithin-PLGA/HSA. These data demonstrate that the poloxamer 188-coated PLGA nanoparticles enable delivery of doxorubicin across the blood-brain barrier in the therapeutically effective concentrations.
Background: Intraosseous (IO) access represents a reliable alternative to intravenous vascular access and is explicitly recommended in the current guidelines of the European Resuscitation Council when intravenous access is difficult or impossible. We therefore aimed to study the efficacy of the intraosseous needle driver EZ-IO(R) in the prehospital setting.
Methods: During a 24-month period, all cases of prehospital IO access using the EZ-IO(R) needle driver within three operational areas of emergency medical services were prospectively recorded by a standardized questionnaire that needed to be filled out by the rescuer immediately after the mission and sent to the primary investigator. We determined the rate of successful insertion of the IO needle, the time required, immediate procedure-related complications, the level of previous experience with IO access, and operator's subjective satisfaction with the device.
Results: 77 IO needle insertions were performed in 69 adults and five infants and children by emergency physicians (n=72 applications) and paramedics (n=5 applications). Needle placement was successful at the first attempt in all but 2 adults (one patient with unrecognized total knee arthroplasty, one case of needle obstruction after placement). The majority of users (92%) were relative novices with less than five previous IO needle placements. Of 22 responsive patients, 18 reported pain upon fluid administration via the needle. The rescuers' subjective rating regarding handling of the device and ease of needle insertion, as described by means of an analogue scale (0 = entirely unsatisfied, 10 = most satisfied), provided a median score of 10 (range 1-10).
Conclusions: The EZ-IO(R) needle driver was an efficient alternative to establish immediate out-of-hospital vascular access. However, significant pain upon intramedullary infusion was observed in the majority of responsive patients.
Hintergrund: Seit 1972 wurde am Fachbereich Medizin der Goethe Universität Frankfurt ein Anamnese- und Untersuchungskurs (UkliF=Untersuchungskurs klinischer Fächer) für Studierende des 1. klinischen Semesters durchgeführt. Dieser Kurs war in den letzten Jahren durch die mangelnde Kursstruktur als wenig zufriedenstellend für alle Beteiligten aufgefallen. Die Fragestellung war demnach, ob es möglich ist durch Reorganisation des klinischen Unterrichts bessere klinische Kompetenz bei den Studierenden und damit bessere Ergebnisse in der „objective structured clinical examination” (OSCE) zu erreichen.
Methoden: Zunächst wurde ein Lernzielkatalog erstellt und ein neues Unterrichtskonzept für den UkliF implementiert. Der neue Untersuchungskurs bestand aus drei Teilen: Zu Beginn ging es um Klärung der Anamneseerhebung und praktische Übungen mit Simulationspatienten. Danach wurden an mehreren Kurstagen klar definierte Teile der körperlichen Untersuchung besprochen und geübt, um schließlich Anamnese und körperliche Untersuchung mit echten Patienten zu praktizieren. Das neue Unterrichtskonzept sah darüber hinaus eine didaktische Schulung für alle beteiligten Lehrenden vor. Die randomisiert ausgewählten Dozenten absolvierten eine sechsstündige Schulung (n=14). In dieser wurden das neue Kurskonzept, die Lehrmethoden und Informationen über Simulationspatienten vorgestellt. Nachfolgend wurden didaktische Grundlagen, Tipps und Tricks für aktiven Unterricht und Regeln des Feedbacks besprochen. In dieser Schulung wurde eine mangelnde Standardisierbarkeit der Untersuchungstechniken erkannt, woraufhin strukturierte Untersuchungsvideos erstellt wurden, um im Anschluss einen Aufbau-Workshop mit diesen Videos stattfinden zu lassen. Für diese Dissertation wurde der Unterricht zunächst teilweise nach dem alten und zum Teil nach dem neuen Verfahren durchgeführt. Zur Überprüfung der Effizienz der Maßnahmen wurde die OSCE als veränderungssensitive Methode gewählt. Alle Studierenden wurden nach Ende ihres Untersuchungskurses in einer 6-Stationen-OSCE evaluiert. 60 Studierende wurden randomisiert in Gruppe A (neuer Kurs mit trainierten Dozenten; n= 24) und Gruppe B (alter Kurs; n=36). Um einen zusätzlichen Nutzen durch einen vorbereitenden OSCE-Workshop vor der Prüfung nachzuweisen, wurde Gruppe A nochmals unterteilt in Gruppe A1 ohne OSCE-Workshop (n=10) und Gruppe A2 mit zusätzlichem OSCE-Workshop (n=14).
Ergebnisse: Gruppe A erreichte signifikant bessere Ergebnisse in der OSCE mit einem Median von 65% und Gruppe B mit 54% der maximal erreichbaren Punkte (p<0,001). In der Subgruppenanalyse konnte kein signifikanter Unterschied zwischen Gruppe A1 und A2 gezeigt werden. Die Kursevaluation durch die Studierenden zeigte eine positive Bewertung des neuen Kurskonzept. Die Evaluation der Dozentenschulung ergab eine Bewertung (in Schulnoten) mit einer Gesamtnote von 1,3 (SD 0,5).
Schlussfolgerung: Diese Studie zeigt, dass ein gut strukturierter Kurs mit klar definierten Lernzielen und speziell trainierten Lehrenden die Studierendenleistungen in der OSCE signifikant erhöhen kann. Es konnte kein zusätzlicher Effekt durch einen vorbereitenden OSCE-Workshop gezeigt werden, was bedeuten kann, dass Kompetenz im Umgang mit der Prüfungsform keinen Störfaktor darstellt für das Ergebnis der Verbesserung der Prüfungsleistung in der OSCE.
Letztendlich kann durch einfache Mittel, wie die Umstrukturierung und Reorganisation des klinischen Unterrichts, ein großer Effekt erzielt werden und die Lehre eine deutliche Verbesserung erfahren. Nach der Pilotstudie lässt sich nun das neue Kurskonzept für alle Studierenden einsetzen und so die Professionalität in der medizinischen Lehre an der Goethe-Universität Frankfurt sichern.
QCD at finite temperature and denisty remains intractable by Monte Carlo simulations for quark
chemical potentials m >∼T. It has been a long standing problem to derive effective theories from
QCD which describe the phase structure of the former with controlled errors. We propose a
solution to this problem by a combination of analytical and numerical methods. Starting from
lattice QCD with in Wilson’s formulation, we derive an effective action in terms of Polyakov
loops by means of combined strong coupling and hopping expansions. The theory correctly
reflects the centre-symmetry in the pure gauge limit and its breaking through quarks. It is valid
for heavy quarks and lattices up to Nt ∼ 6. Its sign problem can be solved and we are able to
calculate the deconfinement transition of QCD with heavy quarks for all chemical potentials.
Effect sizes in experimental pain produced by gender, genetic variants and sensitization procedures
(2011)
Background: Various effects on pain have been reported with respect to their statistical significance, but a standardized measure of effect size has been rarely added. Such a measure would ease comparison of the magnitude of the effects across studies, for example the effect of gender on heat pain with the effect of a genetic variant on pressure pain. Methodology/Principal Findings: Effect sizes on pain thresholds to stimuli consisting of heat, cold, blunt pressure, punctuate pressure and electrical current, administered to 125 subjects, were analyzed for 29 common variants in eight human genes reportedly modulating pain, gender and sensitization procedures using capsaicin or menthol. The genotype explained 0–5.9% of the total interindividual variance in pain thresholds to various stimuli and produced mainly small effects (Cohen's d 0–1.8). The largest effect had the TRPA1 rs13255063T/rs11988795G haplotype explaining >5% of the variance in electrical pain thresholds and conferring lower pain sensitivity to homozygous carriers. Gender produced larger effect sizes than most variant alleles (1–14.8% explained variance, Cohen's d 0.2–0.8), with higher pain sensitivity in women than in men. Sensitization by capsaicin or menthol explained up to 63% of the total variance (4.7–62.8%) and produced largest effects according to Cohen's d (0.4–2.6), especially heat sensitization by capsaicin (Cohen's d = 2.6). Conclusions: Sensitization, gender and genetic variants produce effects on pain in the mentioned order of effect sizes. The present report may provide a basis for comparative discussions of factors influencing pain.
PURPOSE: This study investigates the distortion of geometry of catheters and anatomy in acquired U/S images, caused by utilizing various stand-off materials for covering a transrectal bi-planar ultrasound probe in HDR and LDR prostate brachytherapy, biopsy and other interventional procedures. Furthermore, an evaluation of currently established water-bath based quality assurance (QA) procedures is presented.
MATERIAL AND METHODS: Image acquisitions of an ultrasound QA setup were carried out at 5 MHz and 7 MHz. The U/S probe was covered by EA 4015 Silicone Standoff kit, or UA0059 Endocavity balloon filled either with water or one of the following: 40 ml of Endosgel(®), Instillagel(®), Ultraschall gel or Space OAR™ gel. The differences between images were recorded. Consequently, the dosimetric impact of the observed image distortion was investigated, using a tissue equivalent ultrasound prostate phantom - Model number 053 (CIRS Inc., Norfolk, VA, USA).
RESULTS: By using the EA 4015 Silicone Standoff kit in normal water with sound speed of 1525 m/s, a 3 mm needle shift was observed. The expansion of objects appeared in radial direction. The shift deforms also the PTV (prostate in our case) and other organs at risk (OARs) in the same way leading to overestimation of volume and underestimation of the dose. On the other hand, Instillagel(®) and Space OAR™ "shrinks" objects in an ultrasound image for 0.65 mm and 0.40 mm, respectively.
CONCLUSIONS: The use of EA 4015 Silicone Standoff kit for image acquisition, leads to erroneous contouring of PTV and OARs and reconstruction and placement of catheters, which results to incorrect dose calculation during prostate brachytherapy. Moreover, the reliability of QA procedures lies mostly in the right temperature of the water used for accurate simulation of real conditions of transrectal ultrasound imaging.
As important as the intrinsic properties of an individual nervous cell stands the network of neurons in which it is embedded and by virtue of which it acquires great part of its responsiveness and functionality. In this study we have explored how the topological properties and conduction delays of several classes of neural networks affect the capacity of their constituent cells to establish well-defined temporal relations among firing of their action potentials. This ability of a population of neurons to produce and maintain a millisecond-precise coordinated firing (either evoked by external stimuli or internally generated) is central to neural codes exploiting precise spike timing for the representation and communication of information. Our results, based on extensive simulations of conductance-based type of neurons in an oscillatory regime, indicate that only certain topologies of networks allow for a coordinated firing at a local and long-range scale simultaneously. Besides network architecture, axonal conduction delays are also observed to be another important factor in the generation of coherent spiking. We report that such communication latencies not only set the phase difference between the oscillatory activity of remote neural populations but determine whether the interconnected cells can set in any coherent firing at all. In this context, we have also investigated how the balance between the network synchronizing effects and the dispersive drift caused by inhomogeneities in natural firing frequencies across neurons is resolved. Finally, we show that the observed roles of conduction delays and frequency dispersion are not particular to canonical networks but experimentally measured anatomical networks such as the macaque cortical network can display the same type of behavior.
Background: Working conditions of nursery school teachers have not been scrutinized thoroughly in scientific research. Only a few studies have so far examined work-load and strain in this profession. Preferably, subjective perceptions should be corroborated by data that can be quantified more objectively and accurately. The aim of the present observational field study was to evaluate pedagogical staffs' workflow.
Methods: In 2009 eleven educators in a day care centre were observed throughout three complete workdays. A total of 250 working hours were recorded.
Results: An educators' workday lasted on average 07:46:59 h (SD = 01:01:10 h).Within this time span, an average of 02:20:46 h (30.14%, SD = 00:28:07 h) were spent on caring, 01:44:18 h on playing (22.33%, SD = 00:54:12 h), 00:49:37 h on educational work (10.62%, SD = 00:40:09), and only 00:05:38 h on individual child contact (1.21%, SD = 00:04:58 h).
Conclusion: For the first time, educators' workflow in day care centres was studied in real time. Some of the educators' self-reported problems were corroborated. The results of this study form a basis upon which further investigations can be built and measures can be developed for an overall improvement of child care.
Die Reisetagebücher des Geographen Eduard Pechuël-Loesche von der afrikanischen Loango-Küste, die in der Bayerischen Staatsbibliothek in München als Teil seines Nachlasses aufbewahrt werden, decken leider nicht die gesamte Zeitspanne seines Forschungsaufenthaltes ab, weil die mitgebrachten Hefte bei seiner Ankunft (19.8.1874) bereits vollgeschrieben waren und der Nachschub erst Monate später eintraf. Die fortlaufenden Einträge beginnen daher erst mit den Nachträgen für die Zeit ab 24.2.1875 und reichen bis zum Ende seines Aufenthaltes am 5.5.1876. Die Edition konzentriert sich daher auf diesen Zeitabschnitt und begnügt sich für die Hinreise mit einer Zusammenfassung der wichtigsten Stationen.
Bei den Eintragungen handelt es sich meist um Notizen, die nur stichwortartig in oft unvollständigen Sätzen festgehalten worden waren. Die allgemeine Forschungssituation steht im Vordergrund; demgegenüber treten ethnographische Mitteilungen zurück. Die eigentlichen ethnographischen und sprachlichen Aufzeichnungen wurden offensichtlich in einem „Hauptbuch“, das in einem der Tagebücher erwähnt wird, sowie auf separaten Exzerpt-Zetteln eingetragen, die nicht mehr erhalten sind. Dennoch bilden diese unmittelbaren Tagebuchaufzeichnungen eine wesentliche Ergänzung zu den späteren Veröffentlichungen, besonders in Hinblick auf die Stimmungen und emotionalen Belastungen, denen diese Forschungsexpedition in hohem Maße unterworfen war.
Diese bisher vor allem im Ruf des Scheiterns stehende Expedition ist deshalb so interessant, weil sie eine der ersten interdisziplinären Feldforschungsversuche in Afrika gewesen ist. Ihre Ergebnisse überragen diejenigen der übrigen deutschen Afrika-Unternehmen dieser Zeit bei weitem. Sie hatte allerdings mit zahlreichen widrigen Umständen zu kämpfen, die es ihr verwehrten, ihre eigentliche Aufgabe, den Vorstoß ins Landesinnere (möglichst bis zu einer Erreichung der Ostküste) zu verwirklichen.
Je länger man sich mit diesen Tagebuchnotizen beschäftigt und mit den richtigen Fragen an sie herangeht, desto mehr erschließt sich ihr reichhaltiger Inhalt. Sie sind deshalb erst auf den dritten Blick tatsächlich eine wichtige und sehr vielseitige Quelle, die sich nicht ohne Vorwissen und nur in bestimmten Kontexten erschließt. Wegen ihres disparaten Charakters und ihrer Stakkato-artigen Darstellungsform eignen sich die Tagebücher zwar nicht als spannende Unterhaltungslektüre (und deshalb auch nicht als „normale“ Buchveröffentlichung), doch kann man ihr zu sehr unterschiedlichen Themen nützliche und vielseitige Informationen und Einblicke entlocken.
Da Eduard Pechuël-Loesche in seinen Tagebüchern immer wieder die Anfertigung bestimmter Aquarelle erwähnt, war es ein Glücksfall, dass diese Aquarelle aufgespürt werden und 31 davon in dieser Edition veröffentlicht werden konnten.
Der Edition wurde ein biographischer Abriss über Eduard Pechuël-Loesches Biographie und das Kapitel der Herausgeberin „Feldforschungsstreß an der Loango-Küste: Ein Blick in die Tagebücher von Eduard Pechuël-Loesche (1873–1876)“ aus dem Buch von Sylvia M. Schomburg-Scherff und Beatrix Heintze (Hg.), Die offenen Grenzen der Ethnologie. Schlaglichter auf ein sich wandelndes Fach, Frankfurt am Main: Lembeck 2000, vorangestellt.
