Die Nutzung von Computern hat in unserer Gesellschaft während der letzten Jahre stark zugenommen und eine Umkehrung dieses Trends ist derzeit nicht denkbar. Auch in zunehmend mehr Feldern der Medizin wird Computertechnik genutzt, wobei nicht nur die im medizinischen Sektor Beschäftigten, sondern auch die Patienten immer häufiger mit dem Computer als diagnostischem Hilfsmittel konfrontiert werden. Es erscheint naheliegend, daß psychiatrisch kranke Menschen besonders sensibel hierauf reagieren könnten. Menschen richten ihr Handeln an individuellen Leitschienen aus, sie handeln auf der Grundlage von Einstellungen (englisch: attitude). Akzeptanz beschreibt hingegen einen kurzfristigen Prozeß, die Bereitschaft, etwas Neues anzunehmen. Die vorliegende Arbeit beschäftigt sich mit der Frage, welche Rolle die Einstellung zu Computern (‚computer attitude‘) und die Akzeptanz von Computeruntersuchungen bei deren Anwendung an psychiatrischer Patienten spielen. Sie untersucht, ob und wie sich Einstellung und Akzeptanz messen lassen und wodurch sie beeinflusst werden. 160 stationär behandelte psychiatrische Patienten der Klinik für Psychiatrie und Psychotherapie I der J.W. Goethe-Universität beantworteten einen Fragebogen zur Messung der Einstellung gegenüber Computern (GCAS). Ein Teil (N=78) dieser 160 Patienten absolvierte zusätzlich eine umfangreiche Test- und Fragebogenbatterie am Computer und mit herkömmlichen ‚Papierund-Bleistift‘-Verfahren, in deren Verlauf die Akzeptanz der Untersuchung erfaßt wurde. Die Untersuchungszeitpunkte lagen jeweils in enger zeitlicher Nähe zur Aufnahme und zur Entlassung aus der stationären Behandlung. Wiederum ein Teil (N=22) dieser 78 Patienten hatte zwischen Erst- und Zweituntersuchung Gelegenheit, den Computer in einer spielerisch gestalteten Weise als Freizeitinstrument zu nutzen. Die Auswertungen berücksichtigten die Zugehörigkeit zu drei diagnostischen Subgruppen (‚psychotische Störung‘, ‚affektive Störung‘ und ‚andere Störungen‘). Zunächst wurden die Testgütekriterien der selbst entwickelten Instrumente geprüft. Es zeigte sich eine generell positive Einstellung der untersuchten Patienten gegenüber dem Computer (GCAS-Score > 48 bei 76,9% der Patienten). Bezüglich der diagnostischen Subgruppen zeigten sich keine signifikanten Unterschiede. Männliche (Z=2,4; p=0,02), jüngere (R=-0,22; p=0,002) und besser gebildete Patienten (R=0,34; p=0,00001) hatten jedoch eine bessere Einstellung gegenüber Computern. Es fanden sich Unterschiede in der Akzeptanz der Computeruntersuchung in Abhängigkeit von diagnostischer Subgruppe und Einstellung zu Computern. Die spielerische Kurzintervention erwies sich als nicht geeignet die Einstellung gegenüber Computern signifikant zu verändern. In der vorliegenden Arbeit wird gezeigt, welchen Einfluß Einstellungen und Akzeptanz auf die Interaktion von Patient und Computer haben. Die Kenntnis dieses Einflusses ist für die Bewertung der am Computer erzielten Testergebnisse eine wesentliche Information.
Hintergrund
In Anbetracht ihres bedeutenden Potenzials zur Verbesserung der medizinischen Versorgung wird Telemedizin weiterhin zu wenig genutzt. Trotz einiger erfolgreicher Pilotprojekte in den vergangenen Jahren ist insbesondere über die Hindernisse der Etablierung und Verstetigung von Telemedizin wenig bekannt. Diese Studie hatte das Ziel, die Einstellung niedergelassener Neurologen hinsichtlich der Nutzung von Telemedizin in der Epileptologie und resultierende Hinderungsgründe zu verstehen. Gleichzeitig werden mögliche Lösungsansätze präsentiert.
Methoden
Mithilfe eines individuell erstellten 14-Item-Fragebogens befragten wir prospektiv alle Neurologen, die zuvor die Teilnahme an einem transregionalen Telemedizinpilotprojekt im Bereich der Epileptologie abgelehnt oder keine Rückmeldung gegeben hatten, zu Gründen für und gegen den generellen Einsatz von bzw. die Teilnahme an Telemedizin.
Ergebnisse
Von 58 kontaktierten Neurologen antworteten 33 (57 %). Die häufigsten Gründe für die fehlende Nutzung der Telemedizin waren ein vermuteter Zeitmangel oder ein vermuteter zu großer organisatorischer Aufwand (49 %). Zudem wurden Bedenken bezüglich der technischen Ausstattung (30 %) und eine Präferenz für alternative Wege der intersektoralen Kommunikation (30 %) angegeben. Befürchtete Probleme in Bezug auf die Kostenerstattung für telemedizinische Leistungen waren für 27 % ein Hindernis. Neurologen in ländlichen Gebieten waren signifikant häufiger bereit, zunächst eine telemedizinische Konsultation anzufordern, bevor sie eine Überweisung ausstellen (p = 0,006).
Schlussfolgerungen
Die flächendeckende Etablierung von Telemedizinstrukturen ist immer noch durch Hindernisse erschwert, die meist im organisatorischen Bereich liegen. Die bestehenden Herausforderungen im Gesundheitswesen in ländlichen Gebieten sind eine besondere Chance für die Implementierung von Telemedizin. Die meisten Probleme der Telemedizin können gelöst werden, sollten aber bereits bei der Konzeptionierung von Projekten mitbedacht werden, um ihre Verstetigung zu erleichtern.
Electrical stimulation (EStim) has been shown to promote bone healing and regeneration both in animal experiments and clinical treatments. Therefore, incorporating EStim into promising new bone tissue engineering (BTE) therapies is a logical next step. The goal of current BTE research is to develop combinations of cells, scaffolds, and chemical and physical stimuli that optimize treatment outcomes. Recent studies demonstrating EStim’s positive osteogenic effects at the cellular and molecular level provide intriguing clues to the underlying mechanisms by which it promotes bone healing. In this review, we discuss results of recent in vitro and in vivo research focused on using EStim to promote bone healing and regeneration and consider possible strategies for its application to improve outcomes in BTE treatments. Technical aspects of exposing cells and tissues to EStim in in vitro and in vivo model systems are also discussed.
Electrical stimulation shifts healing/scarring towards regeneration in a rat limb amputation model

Different species respond differently to severe injury, such as limb loss. In species that regenerate, limb loss is met with complete restoration of the limbs’ form and function, whereas in mammals the amputated limb’s stump heals and scars. In in vitro studies, electrical stimulation (EStim) has been shown to promote cell migration, and osteo- and chondrogenesis. In in vivo studies, after limb amputation, EStim causes significant new bone, cartilage and vessel growth. Here, in a rat model, the stumps of amputated rat limbs were exposed to EStim, and we measured extracellular matrix (ECM) deposition, macrophage distribution, cell proliferation and gene expression changes at early (3 and 7 days) and later stages (28 days). We found that EStim caused differences in ECM deposition, with less condensed collagen fibrils, and modified macrophage response by changing M1 to M2 macrophage ratio. The number of proliferating cells was increased in EStim treated stumps 7 days after amputation, and transcriptome data strongly supported our histological findings, with activated gene pathways known to play key roles in embryonic development and regeneration. In conclusion, our findings support the hypothesis that EStim shifts injury response from healing/scarring towards regeneration. A better understanding of if and how EStim controls these changes, could lead to strategies that replace scarring with regeneration.
The antineoplastic alkaloid ellipticine is a prodrug, the pharmacological efficiency of which is dependent on its cytochrome P450 (CYP)- and/or peroxidase-mediated activation to species forming DNA adducts in target tissues. Here, we found that this compound is cytotoxic to human BHT-101, B-CPAP and 8505-C thyroid cancer cells and blocks one or more phases of cell cycle in these cancer cells. Ellipticine toxicity to the thyroid cancer cells corresponded to levels of DNA adducts generated by the CYP- and/or peroxidase-mediated ellipticine metabolites, 12-hydroxy- and 13-hydroxyellipticine, in these cells. Cultivation of all tested cells under hypoxic conditions (1 % oxygen) led to a decrease in ellipticine toxicity. Such a lower sensitivity of cells to ellipticine correlates with a decrease in the formation of ellipticine-derived DNA adducts in these cells. Using Western blotting, the expression of CYP1A1, 1B1, 3A4, thyroid peroxidase (TPO), cyclooxygenase-1 (COX-1) and cytochrome b5, the enzymes that catalyze, and/or influence ellipticine metabolism, was investigated in the cancer cells. Furthermore, the effects of ellipticine treatment on the expression levels of these proteins in thyroid cancer cells were also examined. The results indicate that the highest expression levels of cytochrome b5 together with CYP1A1 and 3A4 determine the highest DNA adduct formation and cytotoxicity of ellipticine in B-CPAP cells. They also demonstrate that formation of covalent DNA adducts by ellipticine is the predominant mechanism responsible for its cytotoxicity in studied cells.
Gene therapy (GT) is becoming a realistic treatment option for patients with haemophilia. Outside clinical trials, the complexity and potential complications of GT will pose unprecedented challenges to haemophilia care centres.AIM: To explore the potential use of electronic tools to improve the delivery of GT under real-world conditions.METHODS: Considering the hub-and-spoke model, the GTH working group on GT considered the entire patient pathway and reached consensus on requirements for an integrative software tool to secure documenting and sharing information between treaters, pharmacies and patients.RESULTS: Six steps of the gene therapy process were identified, each requiring completion of the previous step as a prerequisite for entry. The responsibilities of GT dosing and follow-up treatment centres, read/write access rules, and the minimum data set were outlined. Data contributed by patients through mobile devices was also considered.CONCLUSION: Important information needs to be shared between patients and treatment centres in a real-world GT hub-and-spoke model. Collecting and sharing this information in well-organised electronic applications will not only improve patient care but also enable national and international data collection in clinical registries...
Purpose: Amblyopia with eccentric fixation, especially when not diagnosed early, is a therapeutic challenge, as visual outcome is known to be poorer than in amblyopia with central fixation. Consequently, treatment after late diagnosis is often denied. Electronic monitoring of occlusion provides us the chance to gain first focussed insight into age-dependent dose response and treatment efficiency, as well as the shift of fixation in this rare group of paediatric patients. Methods: In our prospective pilot study, we examined amblyopes with eccentric fixation during 12 months of occlusion treatment. We evaluated their visual acuity, recorded patching duration using a TheraMon®-microsensor, and determined their fixation with a direct ophthalmoscope. Dose-response relationship and treatment efficiency were calculated. Results: The study included 12 participants with strabismic and combined amblyopia aged 2.9–12.4 years (mean 6.5). Median prescription of occlusion was 7.7 h/day (range 6.6–9.9) and median daily received occlusion was 5.2 h/day (range 0.7–9.7). At study end, median acuity gain was 0.6 log units (range 0–1.6) and residual interocular visual acuity difference (IOVAD) 0.3 log units (range 0–1.8). There was neither significant acuity gain nor reduction in IOVAD after the 6th month of treatment. Children younger than 4 years showed best response with lowest residual IOVAD at study end. Efficiency calculation showed an acuity gain of approximately one line from 100 h of patching in the first 2 months and half a line after 6 months. There was a significant decline of treatment efficiency with age (p = 0.01). Foveolar fixation was achieved after median 3 months (range 1–6). Three patients (> 6 years) did not gain central fixation. Conclusion: Eccentric fixation is a challenge to therapy success. Based on electronic monitoring, our study quantified for the first time the reduction of treatment efficiency with increasing age in amblyopes with eccentric fixation. Despite some improvement in patients up to 8 years, older patients showed significantly lower treatment efficiency. In younger patients with good adherence, despite poor initial acuity, central fixation and low residual IOVAD could be attained after median 3 months. Hence, the necessity of early diagnosis and intensive occlusion should be emphasized.

Elektrische Kardioversion bei persistierendem Vorhofflimmern : ambulante versus stationäre Therapie

Aufgrund ihrer bekannten Sicherheit und hohen Erfolgsrate ist die Elektrokardioversion im stationären Rahmen eine etablierte Therapie bei chronisch persistierendem Vorhofflimmern. In der vorliegenden Arbeit haben wir untersucht, ob auch die ambulante Durchführung einer Kardioversion mit gleicher Effektivität und Sicherheit möglich ist. Somit war Ziel der Arbeit, ambulante Kardioversionen zu untersuchen und der stationären Behandlung gegenüberzustellen. Während der anderthalbjährigen Laufzeit der prospektiven, nicht randomisierten Studie wurden 88 konsekutive, elektive Kardioversionen (an 65 Patienten) ambulant durchgeführt; insgesamt wurden während dieses Zeitraums 115 Prozeduren (bei 88 Patienten) erfaßt. Es wurde anschließend eine Nachbeobachtung über einen mittleren Zeitraum von 203 Tagen durchgeführt. Die Studie ergab, daß ambulante Kardioversionen zu 84% erfolgreich waren und Rezidive in 59% der Fälle innerhalb der Nachbeobachtung eintraten. Es zeigte sich bei den Patienten eine Abhängigkeit des Kardioversionserfolges vom Bestehen des Vorhofflimmerns. In der statistischen Analyse stellten sich keine klinischen Parameter als signifikant mit einem Rezidiv verbunden dar. Der abschließende Vergleich mit einer stationär therapierten Patientengruppe ergab keine signifikanten Unterschiede bezüglich Erfolg, Rezidiv und Sicherheit der Behandlung. Behandlungsbedürftige Akutkomplikationen traten bei 1,7% der ambulanten Kardioversionen auf. Schwere Komplikationen, insbesondere Embolien, wurden nicht beobachtet. Es traten keine schweren Blutungskomplikationen bei den chronisch antikoagulierten Patienten auf. Ambulante Kardioversionen sind der Therapie im stationären Rahmen gleichwertig und zudem deutlich kostengünstiger durchführen. Als Konsequenz dieser Untersuchung ist bei elektiven Eingriffen die ambulante Kardioversion des Vorhofflimmerns der stationären Behandlung vorzuziehen.
