Fischöle (n-3 PUFAs) haben potente antientzündliche Wirkungen und klinische Effekte bei chronischen kardialen Erkrankungen. Ebenso wurden positive Effekte auf die bronchiale Entzündung beschrieben. Ziel unserer Studie war es daher zu prüfen, ob eine alleinige fischölreiche Ernährung ohne zusätzliche PUFA-Supplementation messbare Auswirkungen einerseits auf den Lipidstatus im Blut der Patienten, andererseits auf das Ausmaß der nasalen und bronchialen Beschwerden sowie der bronchialen Entzündungsaktivität bei Graspollenallergikern aufweisen kann. Hierzu untersuchten wir 100 Patienten hinsichtlich ihrer nasalen und bronchialen Allergiesymptomatik, erfassten die Ernährungsgewohnheiten mittels eines standardisierten Fragebogens, und bestimmten im Blut den Lipidstatus (Gehalt an mehrfach ungesättigten Fettsäuren; n-3 PUFA). Wir konnten zeigen, dass die Ernährung einen messbaren Einfluss auf den Gehalt an n-3 PUFA im Blut hat und dass dieser Gehalt bei Patienten mit regelmäßigem Fischkonsum höher lag als bei denjenigen, die keinen Fisch zu sich nahmen (Plasma-EPA-Konzentration: Fischesser 1,01 ± 0,7 wt%, Fischhasser 0,28 ± 0,1 wt%; p< 0,004; Plasma-DHA-Konzentration: Fischesser 2,30 ± 0,6 wt%, Fischhasser: 1,19 ± 0,3 wt%; p< 0,0001). Der Vergleich der Fettsäurewerte unserer Patienten mit den Werten von Milbenallergikern unter fünfwöchiger Fischöl-Supplementation machte allerdings deutlich, dass der n-3 PUFA-Gehalt mit gezielter Nahrungsergänzung noch weiter ansteigen kann. Die möglicherweise durch hohe n-3 PUFA-Konzentrationen bedingten protektiven Effekte auf die allergische Atemwegsentzündung der Patienten wurden an der Höhe des eNO als einem spezifischen und sensiblen Marker gemessen. Es konnte gezeigt werden, dass Patienten, die viel Fisch zu sich nahmen, durchschnittlich einen niedrigeren eNO- Ausgangswert entsprechend einer geringeren Entzündung aufwiesen als Patienten ohne Fischkonsum (Fischesser: 21,38 ± 11,8 ppB; Fischhasser: 31,35 ± 19,8; p=0,188). Allerdings unterschied sich der mittlere eNO-Anstieg 24 Stunden nach Graspollenprovokation nicht zwischen Patienten mit Fischkonsum (18,25 ± 23,7 ppB) und Patienten ohne Fischkonsum (19,53 ± 31,6 ppB). Ebenso zeigte sich kein signifikanter Einfluss der PUFA-Spiegel auf das Gesamt-IgE, das spezifische IgE gegen Graspollen und den FEV1-Abfall nach bronchialer Allergenprovokation. Zusammengefasst macht unsere Untersuchung deutlich, dass eine alleinige Ernährung ohne Supplementation keinen signifikanten Effekt auf allergische und entzündliche Parameter bei Graspollenallergikern und milden Asthmatikern besitzt.
GrassVeg.DE – die neue kollaborative Vegetationsdatenbank für alle Offenlandhabitate Deutschlands

Der Bericht stellt die neue kollaborative Vegetationsdatenbank GrassVeg.DE (EU-DE-020; http://bit.ly/2qgX208) vor, die Vegetationsaufnahmen von Grasländern und anderen nicht-aquatischen Offenlandhabitaten Deutschlands sammelt, um sie national und international für die vegetationsökologische Forschung zur Verfügung zu stellen. GrassVeg.DE trägt die Daten zum European Vegetation Archive (EVA) und künftig auch zur globalen Vegetationsdatenbank „sPlot“ bei. Datenlieferanten von GrassVeg.DE behalten volle Verfügungsgewalt über ihre Daten und werden Mitglied des GrassVeg.DE-Konsortiums. Dadurch profitieren sie durch Co-Autorenschaften und Zitate von ihren Beiträgen und erlangen zugleich die Möglichkeit, selbst Projekte zu beantragen, die GrassVeg.DE- oder EVA-Daten nutzen. Die schnell wachsende GrassVeg.DE-Datenbank umfasste im Juli 2017 3.181 Vegetationsaufnahmen aus acht deutschen Bundesländern. Perspektivisch kann GrassVeg.DE dazu beitragen, eine konsistente Neuklassifikation der Graslandvegetationstypen Deutschlands im Rahmen der Synopsis der Pflanzengesellschaften Deutschlands zu ermöglichen. Wir schließen den Beitrag mit einem Aufruf, eigene und aus der Literatur digitalisierte Vegetationsaufnahmen zu GrassVeg.DE beizutragen.
Background In melanoma, preclinical data suggest a possible role of polyunsaturated fatty acids inhibiting cell growth. A new target molecule for free fatty acids, the G protein-coupled receptor GPR40, was identified in melanoma cells.
Objectives The aim of this study was to investigate GPR40 expression in human melanocytic tissues and to evaluate its potential as a prognostic marker.
Methods and Results A total of 114 tissue sections of naevi, primary melanoma and melanoma metastasis were immunohistochemically stained with anti-GPR40. The staining was evaluated, using the immunoreactivity scoring system. Compared to naevi, primary melanoma and melanoma metastasis showed significantly higher levels of GPR40 (P < 0.05). In primary melanoma, GPR40 expression positively correlated with tumour thickness (P = 0.044) and AJCC level (P = 0.017) and in melanoma metastasis with AJCC level (P = 0.035). Primary melanoma patients with high levels of GPR40 had a significantly poorer overall survival (P = 0.004) and shorter disease-free survival (0.040).
Conclusion The present study identified GPR40 as a novel target molecule in melanoma. First evidence for a potential role of the receptor in tumour progression and metastases was found, and it could be demonstrated that GPR40 expression is negatively correlated with patient’s survival.
Background: Transfusion of red blood cell concentrate can be life-saving, but requires accurate dose calculations in children. Aims: We tested the hypothesis that cognitive aids would improve identification of the correct recommended volumes and products, according to the German National Transfusion guidelines, in pediatric transfusion scenarios. Methods: Four online questionnaire-based scenarios, two with hemodynamically stable and two with hemodynamically unstable children, were sent to German and international pediatric anesthetists for completion. In the two stable scenarios, participants were given pre-filled tables that contained all required information. For the two emergency scenarios, existing algorithms were used and required calculation by the user. The results were classified into three categories of deviations from the recommended values (DRV): DRV120 (<80% or >120%), as the acceptable variation; DRV 300 (<33% or >300%), the deviation of concern for potential harm; and DRV 1000 (<10% or >1000%), the excessive deviation with a high probability of harm. Results: A total of 1.458 pediatric anesthetists accessed this simulation questionnaire, and 402 completed questionnaires were available for analysis. A pre-filled tabular aid, avoiding calculations, led to a reduction in deviation rates in the category of DRV120 by 60% for each and of DRV300 by 17% and 20%, respectively. The use of algorithms as aids for unstable emergencies led to a reduction in the deviation rate only for DRV120 (20% and 15% respectively). In contrast, the deviation rates for DRV300 and DRV1000 rose by 37% and 16%, respectively. Participants used higher transfusion thresholds for the emergency case of a 2-year-old compromised child than for the stable case with a patient of the same age (on average, 8.6 g/dL, 95% CI 8.5–8.8 versus 7.1 g/dL, 95% CI 7.0–7.2, p < 0.001) if not supported by our aids. Participants also used a higher transfusion threshold for unstable children aged 3 months than for stable children of the same age (on average, 8.9 g/dL, 95% CI 8.7–9.0 versus 7.9 g/dL, 95% CI 7.7–8.0, p < 0.001). Conclusions: The use of cognitive aids with precalculated transfusion volumes for determining transfusion doses in children may lead to improved adherence to published recommendations, and could potentially reduce dosing deviations outside those recommended by the German national transfusion guidelines.
