Ziele: Das Ziel dieser offiziellen Leitlinie, die von der Deutschen Gesellschaft für Gynäkologie und Geburtshilfe (DGGG) und der Deutschen Krebsgesellschaft (DKG) publiziert und koordiniert wurde, ist es, die Früherkennung, Diagnostik, Therapie und Nachsorge des Mammakarzinoms zu optimieren.
Methoden: Der Aktualisierungsprozess der S3-Leitlinie aus 2012 basierte zum einen auf der Adaptation identifizierter Quellleitlinien und zum anderen auf Evidenzübersichten, die nach Entwicklung von PICO-(Patients/Interventions/Control/Outcome-)Fragen, systematischer Recherche in Literaturdatenbanken sowie Selektion und Bewertung der gefundenen Literatur angefertigt wurden. In den interdisziplinären Arbeitsgruppen wurden auf dieser Grundlage Vorschläge für Empfehlungen und Statements erarbeitet, die im Rahmen von strukturierten Konsensusverfahren modifiziert und graduiert wurden.
Empfehlungen: Der Teil 1 dieser Kurzversion der Leitlinie zeigt Empfehlungen zur Früherkennung, Diagnostik und Nachsorge des Mammakarzinoms: Der Stellenwert des Mammografie-Screenings wird in der aktualisierten Leitlinienversion bestätigt und bildet damit die Grundlage der Früherkennung. Neben den konventionellen Methoden der Karzinomdiagnostik wird die Computertomografie (CT) zum Staging bei höherem Rückfallrisiko empfohlen. Die Nachsorgekonzepte beinhalten Untersuchungsintervalle für die körperliche Untersuchung, Ultraschall und Mammografie, während weiterführende Gerätediagnostik und Tumormarkerbestimmungen bei der metastasierten Erkrankung Anwendung finden.
Purpose: The aim of this official guideline coordinated and published by the German Society for Gynecology and Obstetrics (DGGG) and the German Cancer Society (DKG) was to optimize the screening, diagnosis, therapy and follow-up care of breast cancer.
Methods: The process of updating the S3 guideline dating from 2012 was based on the adaptation of identified source guidelines which were combined with reviews of evidence compiled using PICO (Patients/Interventions/Control/Outcome) questions and the results of a systematic search of literature databases and the selection and evaluation of the identified literature. The interdisciplinary working groups took the identified materials as their starting point to develop recommendations and statements which were modified and graded in a structured consensus procedure.
Recommendations: Part 1 of this short version of the guideline presents recommendations for the screening, diagnosis and follow-up care of breast cancer. The importance of mammography for screening is confirmed in this updated version of the guideline and forms the basis for all screening. In addition to the conventional methods used to diagnose breast cancer, computed tomography (CT) is recommended for staging in women with a higher risk of recurrence. The follow-up concept includes suggested intervals between physical, ultrasound and mammography examinations, additional high-tech diagnostic procedures, and the determination of tumor markers for the evaluation of metastatic disease.
The promotion and extension of continuous cover mixed stands with a simultaneous reduction of conifer-monocultures play a major role in current silvicultural practices in Central Europe. It is assumed that the admixture of the natural dominant beech (Fagus sylvatica) in pure non site-specific conifer stands automatically indicates better conditions in terms of nature conservation and forest management. To test this hypothesis three different conifer-beech-comparisons of pure and mixed stands in Lower Saxony are studied, analyzing plant species diversity and naturalness of understory vegetation as one important indicator for the ecological status of forests. Each comparison includes pure coniferous stands (Picea abies, Pinus sylvestris, Pseudotsuga menziesii), mixed coniferous-beech-stands, and pure beech stands on similar acidic mineral soils where the potential natural vegetation will be an oligotrophic beech forest (L u z u l o - Fa g e t um). The age of stands varies between 50 and 150 years. To specify tree species influence on site conditions and vegetation, the study also includes light climate and soil data of the stands. It is observed that, with regard to all comparisons, the admixture of beech reduces plant species diversity but increases naturalness of the stands. The intensity of beech admixture effects differs. While in Scots pine stands the impact of admixed beech is very noticeable, with the mixed stands being nearly identical with pure beech stands, the species change in Douglas-fir and Norway spruce stands proceeds more slowly. Assuming that the status in nature conservation and forest management is improving with increasing plant species diversity and increasing naturalness, the results of this study show a contrary development on a stand scale, as the potential natural vegetation of the L u z u l o - F a g e t u m is in its self very species poor on vascular plants.
his contribution aims to give a basic overview of the latest results regarding the production of resonances in different collision systems. The results were extracted from experimental data collected with HADES that is a multipurpose detector located at the GSI Helmholtzzentrum, Darmstadt. The main points discussed here are: the properties of the strange resonances Λ(1405) and Σ(1385), the role of Δ’s as a source of pions in the final state, the production dynamics reflected in form of differential cross sections, and the role of the ϕ meson as a source for K− particles.
Herb induced liver injury (HILI) is a particular challenge that also applies to purported cases presumably caused by black cohosh (BC), an herb commonly used to treat menopausal symptoms. We analyzed and reviewed all published case reports and spontaneous reports of initially alleged BC hepatotoxicity regarding quality of case details and causality assessments. Shortcomings of data quality were more evident in spontaneous reports of regulatory agencies compared to published case reports, but assessments with the scale of CIOMS (Council for the International Organizations of Sciences) or its updated version revealed lack of causality for BC in all cases. The applied causality methods are structured, quantitative, and liver specific with clear preference over an ad hoc causality method or the liver unspecific Naranjo scale. Reviewing the case data and the reports dealing with quality specifications of herbal BC products, there is general lack of analysis with respect to authentication of BC in the BC products used by the patients. However, in one single regulatory study, there was a problem of BC authentication in the analysed BC products, and other reports addressed the question of impurities and adulterants in a few BC products. It is concluded that the use of BC may not exert an overt hepatotoxicity risk, but quality problems in a few BC products were evident that require additional regulatory quality specifications.
Ein Ziel des modernen Waldbaus in Mitteleuropa ist die Förderung von Mischbeständen anstelle von Reinbeständen. Auf basenreichen Standorten sollen daher an Stelle von Buchen-Reinbeständen Mischbestände aus Buche (Fagus sylvatica) und Edellaubhölzern entwickelt werden. Diese Wälder zeichnen sich durch eine artenreiche und produktive Krautschicht aus, die an vielen Ökosystemfunktionen entscheidend beteiligt ist. Ob mit einer Zunahme der Baumarten-Diversität eine Veränderung der Krautschicht-Diversität verbunden ist und welche kausalen Mechanismen möglicherweise dafür verantwortlich sind, wurde daher an den naturnah bewirtschafteten Buchen-Edellaubholz-Mischbeständen im Göttinger Wald (Süd-Niedersachsen) untersucht. Auf 63 Untersuchungsflächen (je 400 m²) mit einem Gradienten von nur einer Baumart (nur Buche) mit bis zu fünf Baumarten in der Baumschicht wurde die Artenzusammensetzung, Diversität und Produktivität der Krautschicht bestimmt. Als Umweltfaktoren wurden der pH-Wert und die PAR-Transmissivität direkt gemessen, die Wasser- und Stickstoffversorgung über die Zeigerwerte nach Ellenberg indirekt eingeschätzt. In den Kalk-Buchenwäldern (H o r d e l y m o - F a g e t u m l a t h y r e t o s um) konnten keine signifikanten Zusammenhänge zwischen der Diversität der Baumschicht und der Diversität und Produktivität der Krautschicht festgestellt werden. Mit zunehmendem Lichtangebot erhöhte sich die Diversität der Krautschicht. Gleichzeitig korrelierte die oberirdische Biomasse der Krautschicht positiv mit dem Shannon-Index der Krautschicht. Mit zunehmendem Buchen-Anteil sank der pH-Wert im oberen Mineralboden, ohne dass dies Auswirkungen auf die Diversität und Produktivität der Krautschicht hatte. Auch die kleinräumigen Unterschiede im Lichtangebot und der Basenversorgung hatten keinen Einfluss auf die Krautschicht-Diversität. Einzelne Krautschichtarten zeigten Beziehungen zur Baumarten-Diversität und zu den Umweltfaktoren, allerdings waren diese meist nur schwach ausgeprägt. Die Untersuchungsergebnisse unterscheiden sich deutlich von Ergebnissen aus dem Nationalpark Hainich, wo in Laubwäldern ein enger Zusammenhang zwischen der Baumarten- und Krautschicht-Diversität nachgewiesen wurde. Mögliche Ursachen für diese gegensätzlichen Resultate können in der Methodik (Probeflächenauswahl), der Standorthomogenität (insbesondere die breitere Spanne in der Lössbedeckung und dem Tonanteil in den Böden des Hainichs), der Waldgeschichte und Bewirtschaftung sowie den beteiligten Mischbaumarten (insbesondere in der Rolle der Linde) liegen. Zusätzliche vergleichende und experimentelle Untersuchungen sind daher notwendig, um unser Wissen über die Zusammenhänge zwischen Diversität und Ökosystemfunktionen in Wäldern zu verbessern.
Background: Alzheimer's disease is a common debilitating dementia with known heritability, for which 20 late onset susceptibility loci have been identified, but more remain to be discovered. This study sought to identify new susceptibility genes, using an alternative gene-wide analytical approach which tests for patterns of association within genes, in the powerful genome-wide association dataset of the International Genomics of Alzheimer's Project Consortium, comprising over 7 m genotypes from 25,580 Alzheimer's cases and 48,466 controls.
Principal findings: In addition to earlier reported genes, we detected genome-wide significant loci on chromosomes 8 (TP53INP1, p = 1.4×10−6) and 14 (IGHV1-67 p = 7.9×10−8) which indexed novel susceptibility loci.
Significance: The additional genes identified in this study, have an array of functions previously implicated in Alzheimer's disease, including aspects of energy metabolism, protein degradation and the immune system and add further weight to these pathways as potential therapeutic targets in Alzheimer's disease.
Anhand von Vergleichen vegetationskundlicher Aufnahmen aus den Jahren 1966-1968 und 1999-2000 werden die zeitlichen Veränderungen der Vegetation in bodensauren Buchenwäldern und Fichtenbeständen des Solling (Niedersachsen) untersucht. Besonders für die Fichtenbestände lässt sich dabei ein Rückgang im Deckungsgrad der Bodenvegetation bei gleichzeitiger Zunahme der Artenzahlen pro Fläche belegen. Arten der Waldschläge und -verlichtungen sind neu zu der ursprünglichen Artenkombination der Bestände hinzugekommen. Steigende Tendenzen bei den mittleren Stickstoffzahlen belegen eine Zunahme von Stickstoffzeigern in der Bodenvegetation von Buchen- und Fichtenbeständen, während mit den Säurezeigern die Trennarten des Luzulo-Fagetum abgenommen haben. In Zusammenhang mit diesen anthropogenen Veränderungen der Bodenvegetation durch Eutrophierung und Kalkung wird die zukünftige Entwicklung mitteleuropäischer bodensaurer Buchenwaldgesellschaften diskutiert.
