Korrelation zwischen klinischen, histologischen und apparativen Befunden bei 40 Patienten mit dilatativer Kardiomyopathie

  • Im Rahmen dieser Arbeit werden Patienten mit DCMP einer Myokardbiopsie mit histologischer und molekularbiologischer Analyse der Biopsate unterzogen. Es soll die Korrelation zwischen den histologisch-molekularbiologischen Befunden, sowie den klinischen Verläufen und echokardiographischen Befunden gezeigt werden. Die Arbeit ist Teil einer nationalen Multizenterstudie. Bei 38 Patienten mit der Diagnose DCMP bzw. Myokarditis wurde das Myokardbiopsat aus der linken Kammer histologisch untersucht. Dabei wurde bei den Biopsaten auch der direkte Nachweis von kardiotropen Viren mittels PCR veranlasst. Bei 13 Patienten (34%) konnte eine Viruspersistenz nachgewiesen werden. Diese Patienten boten histologisch das Bild einer DCMP in verschiedenen Stadien. Bei den nachgewiesenen Viren handelte es sich in neun Fällen um Parvo-Virus-B19 (69,2%), in zwei Fällen um CMV (15,4%) und jeweils in einem Fall um EBV und CVB3 (7,7%). Bei 25 Patienten lag histologisch das Bild einer DCMP bzw. Endokardfibroelastose vor. Außerdem sind alle Patienten mit einer Virusmyokarditis einem Eradikationsversuch mit hochdosierter Immunglobulin-Therapie unterzogen worden. Dadurch konnte eine Viruselimination in 11 Fällen erreicht werden. Beim Vergleich der klinischen und echokardiographischen Daten der Patienten mit und ohne Viruspersistenz kann bezüglich des klinischen Verlaufs ein Unterschied zwischen den beiden Gruppen festgestellt werden. Das Kollektiv der Patienten mit Virusnachweis zeigt im Vergleich zur Kontrollgruppe eine bessere Erholung der linksventrikulären Funktion. Damit war eine diagnostische, therapeutische und prognostische Bedeutung in unserer Kohorte sowohl für den Virusnachweis, als auch für die Therapie mit Immunglobulinen nachweisbar.
  • In the context of this work patients with dilated Cardiomyopathy, bioptically proven in cardiac biopsy, with histological and molecular-biological analysis of the biopsy are submitted. The correlation between the histological-molecular-biological findings, as well as the clinical processes and echocardiographical findings are to be demonstrated. This work is part of a national multi- center- clinical trial. Among 38 patients with the diagnosis DCMP and/ or myocarditis the biopsy of the myocard from the left ventricle was histologically examined. With the biopsy also the direct proof of cardiotrop viruses was arranged by means of PCR. In 13 patients (32%) a virus persistence could be proven. These patients offered histologically the picture of a DCMP in different stages. In each case with the proven viruses it concerned in nine cases Parvo-Virus-B19 (69.2%), in two cases CMV (15.4%) and in one case EBV and CVB3 (7.7%) respectively. Among 25 patients histologically the picture of a DCMP and/or endocardfibroelastosis was present. In addition all patients with a virus myocarditis were submitted to an experimental eradication with high-dose immunglobulin therapy. Thus a viruselimination in 11 cases could be achieved. With the comparison of the clinical and echocardiographical data of the patients with and without virus persistence a difference between the two groups can be determined concerning the clinical process. The collective of the patients with virus proof shows a better recovery of the left ventricular function in comparison to the control's group. Thus a diagnostic, therapeutic and prognostic meaning was provable in our cohort both for the virus proof, and for the therapy with immunglobulin.
