Beta-2 selektive Proteasominhibition als Basis einer synergistischen Wirkung von Ibrutinib und Proteasominhibitoren gegen Myelomzellen

  • Das Multiple Myelom (MM) macht ungefähr 15 % aller hämatologischen Neoplasien aus. Die Einführung neuer Therapieoptionen wie der Hochdosis-Chemotherapie, der immunmodulatorischen Medikamente und der Proteasominhibitoren (PI) haben die Behandlung des MM revolutioniert. Der PI Bortezomib (BTZ) ist zu einem Grundstein der Therapie des MM geworden, aber auch der neu zugelassene PI Carfilzomib (CFZ) wird inzwischen eingesetzt. Trotz dieser Fortschritte treten Resistenzen gegen PIs zu Therapiebeginn, beziehungsweise fast unweigerlich im Verlauf der Therapie auf und das MM bleibt größtenteils unheilbar. Gegenüber PIs resistente Myelomzellen zeigen Merkmale von Pre-Plasmablasten, welche die Proteasominhibition überleben. Um mit einer Therapie auch diese resistenten Zellen zu erreichen, ist es notwendig, die PIs mit Substanzen zu kombinieren, die sich gegen die gesamte B-Zell-Linie richten. Ein neuer Hoffnungsträger in der Behandlung von B-Zell-Leukämien ist der Bruton’s Tyrosinkinase Inhibitor Ibrutinib (IBR). IBR greift die in der gesamten B-Zell-Linie exprimierte Bruton’s Tyrosinkinase an, die als Teil des B-Zell-Rezeptor Signalweges eine Schlüsselrolle bei der Entwicklung und Funktionalität normaler B-Zellen einnimmt. Die zytotoxische Aktivität von IBR in MM korreliert speziell mit der Hemmung des nachgeschalteten Signalwegs NF-κB. Die Kombination von PIs mit IBR wird bereits in klinischen Studien getestet. Allerdings fehlen zurzeit die molekularen Grundlagen für den therapeutischen Erfolg dieser Kombination. Zielsetzung dieser Dissertation war es deshalb die drei PIs BTZ, CFZ und das experimentelle, β2-selektive LU-102 hinsichtlich deren Wirkung auf den NF-κB Signalweg und in Kombination mit IBR zu untersuchen. Der präklinisch erprobte PI LU-102 wurde hier für den Vergleich mit den aktuell in der MM Therapie verwendeten Inhibitoren BTZ und CFZ hinzugezogen, da er im Gegensatz zu diesen sehr selektiv die β2-Untereinheit des Proteasom hemmt und damit einen alternativen Ansatz verfolgt. Es konnte gezeigt werden, dass der β2-selektive PI LU-102 den NF-κB Signalweg hemmt und so die Wirkung von IBR unterstützt. Im Gegensatz dazu aktivierten BTZ und CFZ diesen Signalweg und antagonisierten in diesem Punkt die Wirkung von IBR. Weiterhin konnte veranschaulicht werden, dass BTZ in Kombination mit IBR antagonistisch auf die Zytotoxizität in MM Zelllinien wirkt, während CFZ einen grenzwertigen Synergismus mit IBR bei MM Zelllinien zeigt. Den deutlich stärksten Synergismus in Kombination mit IBR zeigte sich bei LU-102. Übereinstimmend mit diesen Ergebnissen konnte für die Kombination von IBR mit LU-102 im Vergleich zu den Kombinationen mit BTZ oder CFZ eine deutlich stärkere Hemmung des NF-κB Signalweges, eine deutlichere Hemmung des Proliferationsmarkers p-STAT3 und eine robustere Aktivierung der Apoptose-Kaskade beobachtet werden. Zudem konnte nachgewiesen werden, dass LU-102 in Kombination mit IBR in vitro die Resistenz gegenüber BTZ oder CFZ überwindet; dieses Ergebnis ließ sich auch an primären Myelomzellen dreier BTZ-refraktären Patienten reproduzieren. In der Diskussion wurden Hypothesen zum unterschiedlichen Wirkverhalten von BTZ und CFZ sowie LU-102 auf den NF-κB Signalweg entwickelt: BTZ und CFZ aktivieren den NF-κB Signalweg, da diese durch Hemmung der für die Proteolyse des Proteasoms geschwindigkeitsbestimmenden β5-Untereinheit den Abbau von IκB im kompensatorisch aktivierten lysosomalen System fördern. Das β2-selektive LU-102 hingegen lässt die proteasomale Proteolyse quantitativ weitgehend unangetastet und führt somit nicht zum lysosomalen Abbau von IκB. Zudem sorgt LU-102 über einen bisher unbekannten Mechanismus für eine Hemmung des NF-κB Signalweges. Zusammenfassend ist durch die vorliegenden Ergebnisse eine weitere klinische Entwicklung der Kombination von β2-inhibierenden Proteasominhibitoren, wie LU-102 oder Carfilzomib, mit Ibrutinib im Gegensatz zur Kombination von Bortezomib, welches die β2-Aktivität nicht inhibiert, mit Ibrutinib für das Multiple Myelom zu empfehlen.
  • Multiple myeloma (MM) accounts for about 15 % of all hematologic neoplasia. The introduction of new therapy options like high-dose chemotherapy, immunomodulatory agents and proteasome inhibitors (PI) have revolutionized MM therapy. The first-in-class PI Bortezomib (BTZ) has become a backbone of MM therapy, but also the recently approved second-generation PI Carfilzomib (CFZ) is now available. Despite this progress, PI resistance occurs almost inevitably during or at the beginning of PI therapy and thus MM remains incurable for the majority of patients. PI-resistant MM cells show characteristics of pre-plasmablasts which survive proteasome inhibition. Hence, combining proteasome inhibitor therapy with a broadly active B-cell-targeting drug is a promising concept to overcome PI-refractory MM. A new agent in therapy of B-cell-leukemia is the Bruton’s tyrosine kinase (BTK) inhibitor Ibrutinib (IBR). IBR attacks BTK, which is expressed over the entire B-cell development. BTK plays as part of B-cell-receptor pathway a key role during development and functionality of normal B-cells. IBR-related cytotoxicity particularly correlates with inhibition of the downstream pathway NF-κB. The combination of PIs and IBR is currently under investigation in clinical studies. However, the basic molecular principals for therapeutic success of this combination are still lacking. Therefore, the aim of this work was to compare the three PIs BTZ, CFZ and the β2-selective LU-102 with regard to their impact on NF-κB pathway and in combination with Ibrutinib. The pre-clinical PI LU-102 was used in comparison to the established β5-targeting PIs BRZ and CFZ because it selectively inhibits the β2-subunit of the proteasome and thereby follows an alternative approach. In my experiments the β2-selective PI LU-102 inhibited the NF-κB pathway and hence supported the effect of IBR. In contrast, BTZ and CFZ activated this pathway and therefore could antagonize the activity of IBR. Furthermore, BTZ in combination with IBR was antagonistic whereas CFZ reached a borderlinesynergism in combination with IBR. Both results were exceeded by the high synergism of LU-102 in combination with IBR. Consistent with this data the combination of IBR with LU-102 showed superior inhibition of the NF-κB pathway and the proliferation marker p-STAT3 as well as the highest activation of the apoptotic cascade. In addition, I could show that LU-102 in combination with IBR in vitro overcomes BTZ and CFZ resistance; these results have been reproduced in primary MM cells of three BTZ-refractory patients. The discussion develops hypotheses for the different effects on the NF-κB pathway of BTZ and CFZ as well as LU-102: BTZ and CFZ activate the NF-κB pathway because their inhibition of the proteasomal β5-subunit, which is speed-driving for proteasomal proteolysis, leads to IκB degradation in the compensatory lysosomal pathway. In contrast, the β2-selective LU-102 leaves the proteasomal proteolysis quantitatively untouched and hence does not activate the lysosomal pathway of IκB. Moreover, LU-102 inhibits the NF-κB pathway on a yet unknown mechanism. Overall, the data identified β2-inhibiting proteasome inhibitors like LU-102 or Carfilzomib as superior combination partners with Ibrutinib in comparison to Bortezomib, which does not target the β2-activity and suggest a further clinical development of these combinations for multiple myeloma.
