Modulation des Chemokin-Profils beim Prostatakarzinom : neue Ansätze in Diagnostik und Therapie

  • Zahlreiche Studien postulieren einen Einfluss löslicher Chemokine und ihrer Liganden auf die Malignität des Prostatakarzinoms. Es gibt Hinweise, dass die CXC-Untergruppe der Chemokin Familie eine besondere Rolle bei der Progression und Metastasierung des Prostatakarzinoms spielt. Da wenig über das Expressionsprofil der CXC Chemokine (CXCL) und ihrer korrespondierenden Rezeptoren (CXCR) beim Prostatakarzinom bekannt ist, wurde in dieser Arbeit die Erstellung eines Chemokin-Profils angestrebt. An 50 Gewebsproben wurde die Expression der CXCR und CXCL an Karzinomgewebe analysiert und jeweils mit dem Muster im tumorfreien Probenmaterial verglichen. Mittels reverse transcription polymerase chain reaction (RT-PCR) wurde dabei die Genaktivität der Chemokine CXCL1, CXCL2, CXCL4, CXCL5, CXCL8, CXCL9, CXCL10, CXCL12, CXCL13 sowie ihrer Rezeptoren CXCR1-CXCR6 ermittelt. Des Weiteren wurde durch konfokale Laserscanmikroskopie die Verteilung der CXC Rezeptoren an Prostatatumorzellen (LNCaP) evaluiert. Im Vergleich zum Normalgewebe zeigte sich eine veränderte Expression von sowohl CXCL als auch CXCR im Tumorareal. Als bemerkenswert ist die deutlich reduzierte Expression von CXCL13 und CXCR5 mRNA im Tumorgewebe einzuordnen. Hervorzuheben sind ferner Unterschiede im Verhältnis einzelner Chemokine zueinander. Obwohl signifikante Differenzen am Tumormaterial hinreichend belegt werden konnten, trifft dies nicht in gleichem Maße zu, wenn die ermittelten Daten in Relation zum histopathologischen Grading, der Tumorinvasivität und Gleason Score gesetzt wurden. Insgesamt lässt sich ableiten, dass die Progression des Prostatatumors mit Alterationen verschiedener CXC Chemokine einhergeht. Die Modulation eines komplexen Chemokin-Profils ist dabei für die Tumormalignität von größerer Relevanz, als die Veränderung eines einzelnen Chemokins. Die biologische Bedeutung der veränderten Expression ist bislang unklar. Da aber das lokale Wachstum und die lokale Metastasierung des Prostatakarzinoms möglicherweise direkt über Veränderungen des Chemokin-Profils beeinflusst werden, erlaubt die Analyse der CXCL/CXCR-Achse wichtige Schlüsse zur Prognose der Erkrankung. Auch sind innovative Therapiekonzepte denkbar, die in die Regulation der Chemokine eingreifen und so einem progredienten Krankheitsverlauf entgegensteuern.
  • Several studies postulate an influence of soluble chemokines and their receptors on prostate cancer malignancy. It is supposed that CXC-subtypes of the chemokine family play an important role in the progression and metastasis of prostate tumors. Because little is known about the expression profile of the CXC chemokines (CXCL) and corresponding receptors (CXCR) in prostate cancer, the complete chemokine profile was evaluated in the present work. Based on 50 tumor specimen, expression of CXCR and CXCL was analyzed in tumor-containing versus tumor-free tissue. Gene activity of CXCL chemokines CXCL1, CXCL2, CXCL4, CXCL5, CXCL8, CXCL9, CXCL10, CXCL12, CXCL13 and their receptors CXCR1 - CXCR6 was measured by reverse transcription polymerase chain reaction (RT-PCR). Furthermore, localization of CXCR was detected by confocal laser scanning microscopy using the prostate tumor cell line LNCaP. Compared to normal prostate tissue, distinct alterations of CXCL and CXCR became obvious in the tumor specimen. Remarkably, both CXCL13 and CXCR5 mRNAs were drastically reduced in tumor material. Calculation of the relative chemokine ratio revealed distinct alterations between tumor and normal tissue. Although significant differences were seen in cancerous tussue per se, this was not true with respect to the histopathological grading, tumor invasiveness and Gleason Score. The data provide evidence that tumor progression is correlated with CXC-alterations. In this context, the modulation of a complex chemokine profile is of greater relevance, than changes of an individual chemokine. The biological significance of the changes observed is not clear. However, local growth and metastasis might be directly influenced by CXCL and CXCR. In that case, analysis of the CXCL/CXCR axis allows to conclude to prostate cancer progression. Furthermore, novel antitumoral protocols may block tumor migration and metastasis by CXCL/CXCR regulation.
