T1  - Verhalten der Plättchenadhäsivität an silikonisiertem Glas bei Patienten mit elektiven Hüftgelenkersatz unter einer Thromboseprophylaxe mit Hirudin oder Heparin
N2  - In dieser Arbeit wurde im Rahmen einer großen prospektiven, internationalen, multizentrischen, doppelblinden und randomisierten Studie an einer Subgruppe von 44 Patienten mit elektiver Hüftgelenkersatzoperation die Plättchenadhäsivität an silikonisiertem Glas untersucht. Ziel der Gesamtstudie war es, die Effiezienz von drei unterschiedlichen Hirudin-Dosen im Vergleich zu der üblichen Behandlung mit unfraktioniertem Heparin in der antithrombotischen Wirkung zu prüfen. 12 Patienten erhielten 2 x 10 mg, 10 Patienten 2 x 15 mg, 11 Patienten 2 x 20mg Hirudin und 11 Patienten 3 x 5000 IE unfraktioniertes Heparin. Bei allen Patienten wurde am 10. postoperativen Tag eine Phlebographie vorgenommen. Bei 6 Patienten stellte man phlebografisch eine Beinvenenthrombose ohne klinische Symptome fest. Der Plättchenadhäsivitätindex stieg unspezifisch in allen Gruppen am 1.postoperativen Tag an und fiel am 3./4. postoperativen Tag wieder mehr oder weniger gleich stark ab. Präoperativ sowie am 1. und 3./4. postoperativen Tag wurden zusätzlich folgende Parameter bestimmt: aPTT, Heptest, Fibrinogen und F VIII assoziertes Antigen. APTT stieg in beiden Gruppen an. Fibrinogen und F VIII stiegen postoperativ deutlich an. Da aber dieser Anstieg sowohl bei der Thrombosegruppe als auch bei der Nicht-Thrombosegruppe auftrat, hatten auch diese Werte keinen prädiktiven Wert. Der Heptest stieg minimal in beiden Gruppen. Dieser Test ist auch verwendbar zur Überwachung einer Behandlung mit Thrombinhemmern.
N2  - This work investigated the platelet adhesion at siliconized glass in a frame of a big prospective, international, multicentric, double blind and randomised study at a sub group of 44 patients with elective hip joint substitute operation. Target of the study was to check the efficiency concerning to the antithrombotic effect comparing three different hirudin doses with the common treatment with unfractionied heparin. 12 patients received 2 x 10 mg, 10 patients 2 x 15 mg, 11 patients 2x 20 mg hirudin und 11 patients 3 x 5000 IE unfractionied heparin. With all patients was done a phlebografy after the 10th day post operative. With 6 patients was found out a legveintrhrombosis without clinical symptoms. The index of platelet adhesion increased unspecificly in all groups after the 1. day post op and decreased after the 3rd -4th day post operative more or less with the same decrease rate. Pre operative at the 1st and 3rd-4th day post operative determined also the following parameters: aPTT, Heptest, fibrinogen und F V111 associated antigen. aPTT increased in both groups. Fibrinogen and F V111 increased significant post operative. But this has no predicvtive value because this increase was present in the thrombotic group and in the non-thrombotic group. The hep test minimally increased in both groups. This test is also useful for thrombininhibitor-treatment monitoring.
