Introduction: The German PID-NET registry was founded in 2009, serving as the first national registry of patients with primary immunodeficiencies (PID) in Germany. It is part of the European Society for Immunodeficiencies (ESID) registry. The primary purpose of the registry is to gather data on the epidemiology, diagnostic delay, diagnosis, and treatment of PIDs.
Methods: Clinical and laboratory data was collected from 2,453 patients from 36 German PID centres in an online registry. Data was analysed with the software Stata® and Excel.
Results: The minimum prevalence of PID in Germany is 2.72 per 100,000 inhabitants. Among patients aged 1–25, there was a clear predominance of males. The median age of living patients ranged between 7 and 40 years, depending on the respective PID. Predominantly antibody disorders were the most prevalent group with 57% of all 2,453 PID patients (including 728 CVID patients). A gene defect was identified in 36% of patients. Familial cases were observed in 21% of patients. The age of onset for presenting symptoms ranged from birth to late adulthood (range 0–88 years). Presenting symptoms comprised infections (74%) and immune dysregulation (22%). Ninety-three patients were diagnosed without prior clinical symptoms. Regarding the general and clinical diagnostic delay, no PID had undergone a slight decrease within the last decade. However, both, SCID and hyper IgE- syndrome showed a substantial improvement in shortening the time between onset of symptoms and genetic diagnosis. Regarding treatment, 49% of all patients received immunoglobulin G (IgG) substitution (70%—subcutaneous; 29%—intravenous; 1%—unknown). Three-hundred patients underwent at least one hematopoietic stem cell transplantation (HSCT). Five patients had gene therapy.
Conclusion: The German PID-NET registry is a precious tool for physicians, researchers, the pharmaceutical industry, politicians, and ultimately the patients, for whom the outcomes will eventually lead to a more timely diagnosis and better treatment.
Ziele: Das Ziel dieser offiziellen Leitlinie, die von der Deutschen Gesellschaft für Gynäkologie und Geburtshilfe (DGGG) und der Deutschen Krebsgesellschaft (DKG) publiziert und koordiniert wurde, ist es, die Früherkennung, Diagnostik, Therapie und Nachsorge des Mammakarzinoms zu optimieren.
Methoden: Der Aktualisierungsprozess der S3-Leitlinie aus 2012 basierte zum einen auf der Adaptation identifizierter Quellleitlinien und zum anderen auf Evidenzübersichten, die nach Entwicklung von PICO-(Patients/Interventions/Control/Outcome-)Fragen, systematischer Recherche in Literaturdatenbanken sowie Selektion und Bewertung der gefundenen Literatur angefertigt wurden. In den interdisziplinären Arbeitsgruppen wurden auf dieser Grundlage Vorschläge für Empfehlungen und Statements erarbeitet, die im Rahmen von strukturierten Konsensusverfahren modifiziert und graduiert wurden.
Empfehlungen: Der Teil 1 dieser Kurzversion der Leitlinie zeigt Empfehlungen zur Früherkennung, Diagnostik und Nachsorge des Mammakarzinoms: Der Stellenwert des Mammografie-Screenings wird in der aktualisierten Leitlinienversion bestätigt und bildet damit die Grundlage der Früherkennung. Neben den konventionellen Methoden der Karzinomdiagnostik wird die Computertomografie (CT) zum Staging bei höherem Rückfallrisiko empfohlen. Die Nachsorgekonzepte beinhalten Untersuchungsintervalle für die körperliche Untersuchung, Ultraschall und Mammografie, während weiterführende Gerätediagnostik und Tumormarkerbestimmungen bei der metastasierten Erkrankung Anwendung finden.
Purpose: The aim of this official guideline coordinated and published by the German Society for Gynecology and Obstetrics (DGGG) and the German Cancer Society (DKG) was to optimize the screening, diagnosis, therapy and follow-up care of breast cancer.
Methods: The process of updating the S3 guideline dating from 2012 was based on the adaptation of identified source guidelines which were combined with reviews of evidence compiled using PICO (Patients/Interventions/Control/Outcome) questions and the results of a systematic search of literature databases and the selection and evaluation of the identified literature. The interdisciplinary working groups took the identified materials as their starting point to develop recommendations and statements which were modified and graded in a structured consensus procedure.
Recommendations: Part 1 of this short version of the guideline presents recommendations for the screening, diagnosis and follow-up care of breast cancer. The importance of mammography for screening is confirmed in this updated version of the guideline and forms the basis for all screening. In addition to the conventional methods used to diagnose breast cancer, computed tomography (CT) is recommended for staging in women with a higher risk of recurrence. The follow-up concept includes suggested intervals between physical, ultrasound and mammography examinations, additional high-tech diagnostic procedures, and the determination of tumor markers for the evaluation of metastatic disease.
Ein Ausbau der Windenergienutzung auf See könnte zu einem
deutlichen Verlust an störungsfreien Überwinterungs- und
Rastgebieten für Seevögel führen. Um die Auswirkungen von
Windparks vor, während und nach ihrem Bau einschätzen zu
können, werden im derzeitigen Standarduntersuchungskonzept
des Bundesamtes für Seeschifffahrt und Hydrographie
(BSH) neben schiffs- auch flugzeugbasierte Vogelzählungen
empfohlen. Diese visuellen Transektzählungen haben jedoch
methodische Nachteile. Aus der zur Arterkennung erforderlichen
Flughöhe (78 m) und mit entsprechender Fluggeschwindigkeit
können bei hohem Vogelaufkommen lediglich
grobe Bestandskategorien geschätzt werden. Darüber hinaus
üben Flugzeuge in dieser Höhe eine Scheuchwirkung auf
Rastvögel aus, wodurch die Erfassbarkeit einiger Arten (v. a.
Trauerenten) zusätzlich erschwert und die zu untersuchende
Störwirkung von Windparks überlagert wird. In der vorliegenden
Pilotstudie vergleichen wir eine herkömmliche Flugzeugtransektzählung
mit einer kurz zuvor durchgeführten
fotografischen Seevogelerfassung über der Wismarbucht in
der deutschen Ostsee. Die fotografische Kartierung erfolgte
mit einer hoch auflösenden Digitalkamera (39 Megapixel) aus
200 m Höhe. Entlang definierter Transekte wurden in regelmäßigen
Abständen 415 entzerrte, maßstabsgetreue Digitalfotos
(Orthofotos) aufgenommen. Die Ergebnisse dieser
Studie zeigen, dass bei der herkömmlichen Flugzeugtransektzählung
die Bestände von Meeresenten (Eider-, Eis-, Trauerenten)
deutlich unterschätzt wurden. Die Abweichung gegenüber
der fotografischen Methode variierte deutlich zwischen
den Arten. Während des visuellen Zählflugs wurden ohne
Korrekturfaktoren 85 % (Eiderente), 41 % (Eisente) und lediglich
2 % (Trauerente) der fotografisch nachgewiesenen Individuen
erfasst. Die Ursachen für diese quantitativen Unterschiede
werden diskutiert.
Der Zyklus 'Letter of Resignation', entstanden zwischen 1959 und 1967, stellt in verschiedener Hinsicht eine Besonderheit in Cy Twomblys Oeuvre dar. Selten setzt Twombly sich so konkret mit dem Topos der Schrift auseinander, ohne daß dies eingebunden wäre in einen der mythischen Vorwürfe, die viele seiner Bilder und Zyklen bestimmen. Auch die Landschaft rückt zu Gunsten der Beschäftigung mit der Kommunikation mittels Schrift und der Vermittlung von Inhalt durch Schrift auf dem Papier in den Hintergrund.
Der Unterschied von Ausstellungen, bei denen es um Literatur als ein diskursives Konstrukt geht, und solchen, bei denen die materiellen Träger von Literatur, sozusagen die Hardware, ausgestellt wird, kann man mit Hans-Otto Hügel nachvollziehen, der auf eine lange zurückreichende Tradition der Ausstellung von Büchern hinweist. Im Vordergrund steht hier das Buch in seiner Materialität und nicht die in ihm enthaltene Literatur. Mit dem Ausstellen von Büchern ist allerdings noch keine Literaturausstellung geschaffen. Für das Ausstellen von Büchern, so Hans-Otto Hügel, könne eine Frühgeschichte konstatiert werden, die im alten Rom beginne, sich über die Klosterbibliotheken des Mittelalters, die Fürstenbibliotheken der Renaissance, die Stadt- und Universitätsbibliotheken des 16. und 17. Jahrhunderts verfolgen lasse. "Diese Linie führt - nach der Trennung von Magazin- und Leseraum - direkt bis zu den ersten, ausschließlich zum Vorzeigen eingerichteten Vitrinen- und Dauerausstellungen in der ersten Hälfte des 19. Jahrhunderts" (Hügel 1991a, 7). Hinsichtlich einer Geschichte und historischen Genese von Literaturausstellung gäbe es hingegen nur Ausstellungen mit Dichterportraits zu verzeichnen, die lediglich als eine Vor-, aber nicht als Frühgeschichte gelten könnten. Die Geschichte literarischer Ausstellungen, oder besser: die Geschichte der Ausstellung literarischen Materials und literarischer Dokumente, ist somit ein vergleichsweise modernes Phänomen, das vor allem vor dem Hintergrund, respektive im Rahmen der Geschichte und Entwicklung literarischer Archive, Museen und Gedenkstätten zu betrachten ist, die, so Hügel, im Zusammenhang der Popularisierung historischen Denkens, der Suche nach Ansätzen für nationale Identitäten und der geistigen Bedeutung der Wissenschaften im 19. Jahrhundert stünden. Seitdem nehmen die literarischen Ausstellungsorte neben technischen, naturwissenschaftlichen, kulturhistorischen, religiösen, kunstgewerblichen und volkskundlichen Museen und solchen der bildenden Kunst einen festen Platz in der Museumslandschaft ein und werden im Allgemeinen zu den kulturhistorischen Museen gezählt.
Simple Summary: In patients with myeloproliferative neoplasms (MPN) and in patients with kidney dysfunction, a higher rate of thrombosis has been reported compared with the general population. Furthermore, MPN patients are more prone to develop kidney dysfunction. In our study, we assessed the importance of specific risk factors for kidney dysfunction and thrombosis in MPN patients. We found that the rate of thrombosis is correlated with the degree of kidney dysfunction, especially in myelofibrosis. Significant associations for kidney dysfunction included arterial hypertension, MPN treatment, and increased inflammation, and those for thrombosis comprised arterial hypertension, non-excessive platelet counts, and antithrombotic therapy. The identified risk factor associations varied between MPN subtypes. Our data suggest that kidney dysfunction in MPN patients is associated with an increased risk of thrombosis, mandating closer monitoring, and, possibly, early thromboprophylaxis.
Abstract: Inflammation-induced thrombosis represents a severe complication in patients with myeloproliferative neoplasms (MPN) and in those with kidney dysfunction. Overlapping disease-specific attributes suggest common mechanisms involved in MPN pathogenesis, kidney dysfunction, and thrombosis. Data from 1420 patients with essential thrombocythemia (ET, 33.7%), polycythemia vera (PV, 38.5%), and myelofibrosis (MF, 27.9%) were extracted from the bioregistry of the German Study Group for MPN. The total cohort was subdivided according to the calculated estimated glomerular filtration rate (eGFR, (mL/min/1.73 m2)) into eGFR1 (≥90, 21%), eGFR2 (60–89, 56%), and eGFR3 (<60, 22%). A total of 29% of the patients had a history of thrombosis. A higher rate of thrombosis and longer MPN duration was observed in eGFR3 than in eGFR2 and eGFR1. Kidney dysfunction occurred earlier in ET than in PV or MF. Multiple logistic regression analysis identified arterial hypertension, MPN treatment, increased uric acid, and lactate dehydrogenase levels as risk factors for kidney dysfunction in MPN patients. Risk factors for thrombosis included arterial hypertension, non-excessive platelet counts, and antithrombotic therapy. The risk factors for kidney dysfunction and thrombosis varied between MPN subtypes. Physicians should be aware of the increased risk for kidney disease in MPN patients, which warrants closer monitoring and, possibly, early thromboprophylaxis.
Patients with type 2 diabetes (T2D) are threatened by excessive cardiovascular morbidity and mortality. While accelerated arterial stiffening may represent a critical mechanistic factor driving cardiovascular risk in T2D, specific therapies to contain the underlying diabetic arterial remodeling have been elusive. The present translational study investigates the role of microRNA-29b (miR-29b) as a driver and therapeutic target of diabetic aortic remodeling and stiffening. Using a murine model (db/db mice), as well as human aortic tissue samples, we find that diabetic aortic remodeling and stiffening is associated with medial fibrosis, as well as fragmentation of aortic elastic layers. miR-29b is significantly downregulated in T2D and miR-29b repression is sufficient to induce both aortic medial fibrosis and elastin breakdown through upregulation of its direct target genes COL1A1 and MMP2 thereby increasing aortic stiffness. Moreover, antioxidant treatment restores aortic miR-29b levels and counteracts diabetic aortic remodeling. Concluding, we identify miR-29b as a comprehensive—and therefore powerful—regulator of aortic remodeling and stiffening in T2D that moreover qualifies as a (redox-sensitive) target for therapeutic intervention.
A wide variety of enzymatic pathways that produce specialized metabolites in bacteria, fungi and plants are known to be encoded in biosynthetic gene clusters. Information about these clusters, pathways and metabolites is currently dispersed throughout the literature, making it difficult to exploit. To facilitate consistent and systematic deposition and retrieval of data on biosynthetic gene clusters, we propose the Minimum Information about a Biosynthetic Gene cluster (MIBiG) data standard.
Background & Aims: In ACLF patients, an adequate risk stratification is essential, especially for liver transplant allocation, since ACLF is associated with high short-term mortality. The CLIF-C ACLF score is the best prognostic model to predict outcome in ACLF patients. While lung failure is generally regarded as signum malum in ICU care, this study aims to evaluate and quantify the role of pulmonary impairment on outcome in ACLF patients.
Methods: In this retrospective study, 498 patients with liver cirrhosis and admission to IMC/ICU were included. ACLF was defined according to EASL-CLIF criteria. Pulmonary impairment was classified into three groups: unimpaired ventilation, need for mechanical ventilation and defined pulmonary failure. These factors were analysed in different cohorts, including a propensity score-matched ACLF cohort.
Results: Mechanical ventilation and pulmonary failure were identified as independent risk factors for increased short-term mortality. In matched ACLF patients, the presence of pulmonary failure showed the highest 28-day mortality (83.7%), whereas mortality rates in ACLF with mechanical ventilation (67.3%) and ACLF without pulmonary impairment (38.8%) were considerably lower (p < .001). Especially in patients with pulmonary impairment, the CLIF-C ACLF score showed poor predictive accuracy. Adjusting the CLIF-C ACLF score for the grade of pulmonary impairment improved the prediction significantly.
Conclusions: This study highlights that not only pulmonary failure but also mechanical ventilation is associated with worse prognosis in ACLF patients. The grade of pulmonary impairment should be considered in the risk assessment in ACLF patients. The new score may be useful in the selection of patients for liver transplantation.
