Ibuprofen gehört zu den am meisten verwendeten nichtsteroidalen Antiphlogistika (NSAIDs) und ist in niedriger Dosierung von 200-400 mg als sogenanntes OTC (over the counter) Analgetikum frei erhältlich. Obwohl es in den meisten Fällen als razemische Mischung aus S- und R-Ibuprofen verabreicht wird, ist mittlerweile auch das reine S-Ibuprofen (Dexibuprofen) als Medikament erhältlich. Während S-Ibuprofen ein potenter COX-Inhibitor ist, zeigt R-Ibuprofen in den klinisch relevanten Konzentrationen keine COX-Hemmung. Seit längerem ist bekannt, dass NSAIDs bei einer kontinuierlichen Einnahme über mehrere Jahre die Initiierung und Proliferation von Tumoren hemmen. Die Hemmung der Cyclooxygenase wird dabei als ein wichtiger Mechanismus für die antikarzinogene Wirkung angesehen, doch für viele NSAIDs sind auch schon COX unabhängige Mechanismen beschrieben worden. In der vorliegenden Arbeit wurde unter Verwendung zweier Tumorzelllinien mit unterschiedlicher COX-2 Expression und dem Einsatz von beiden Ibuprofen Enantiomeren untersucht, inwieweit COX unabhängige Mechanismen für die antikarzinogenen Effekte von S- und R-Ibuprofen verantwortlich sind. Sowohl die COX-2 defizienten HCT-15 als auch die COX-2 exprimierenden HCA-7 Zellen wurden durch Behandlung mit S- oder R-Ibuprofen in ihrer Proliferaton gehemmt. Dabei führte die Behandlung mit S- und R-Ibuprofen für 24 h zu einem G1-Zellzyklusblock und nach 72 h zu einem signifikanten Anstieg von apoptotischen Zellen, was im Western-Blot Assay durch eine verminderte Cyclin A und B Expression, einer erhöhten Expression des Zellzyklusinhibitors p27KIP1 und PARP-Spaltung bestätigt wurde. Dabei zeigten beide Enantiomere in den Tumorzellen eine gleich starke antiproliferative und apoptotische Wirkung. Eine Messung der intrazellulären S- und R-Ibuprofen Konzentrationen ergab außerdem, dass in den verwendeten Tumorzellen keine unidirektionale Konfigurationsinversion von R- zu S-Ibuprofen stattgefunden hat. In den COX-2 exprimierenden HCA-7 Zellen waren die antikarzinogenen Effekte von S- und R-Ibuprofen schwächer ausgeprägt als in den HCT-15 Zellen, was jedoch auf eine niedrigere intrazelluläre S- bzw. R-Ibuprofen Konzentration zurückzuführen war. Diese Ergebnisse zeigen auf jeden Fall, dass COX unabhängige Mechanismen bei der antikarzinogenen Wirkung von S- und R- Ibuprofen eine Rolle spielen. Dass auch COX abhängige Mechanismen an der antikarzinogenen Wirkung von Ibuprofen beteiligt sind, konnte in verschiedenen Studien gezeigt werden. In der vorliegenden Arbeit wurde eine Abnahme an PGE2 nach S-Ibuprofen Behandlung in den COX-2 exprimierenden Tumorzellen beobachtet. In welchem Ausmaß aber die COX Hemmung an der antikarzinogenen Wirkung von S-Ibuprofen in den COX-2 exprimierenden (Hemmung der PGE2 Synthese) und möglicherweise auch in den COX-2 defizienten Zellen (Anstieg an Arachidonsäure und daraus resultierend an Ceramid) beteiligt war, konnte anhand der Ergebnisse nicht geklärt werden. Es kann jedoch angenommen werden, dass S-Ibuprofen zumindest in den COX exprimierenden HCA-7 Zellen sowohl über COX Hemmung als auch über COX unabhängige Mechanismen antiproliferativ und apoptotisch wirkt. Auch im Tumormodell der Maus wurde bei einer täglichen i.p. Applikation von 15 mg/kg S- oder R-Ibuprofen eine Hemmung des Tumorwachstums beobachtet. Dabei betrug die unidirektionale Inversion von R- zu S-Ibuprofen ca. 54 %. Die Inkubation mit S- bzw. R-Ibuprofen führte in beiden Zelllinien außerdem zu einer Akkumulation des Tumorsuppressors p53, der an der Regulierung von Zellzyklus und Apoptose beteiligt ist. Daher wurde im zweiten Teil der Arbeit untersucht, ob S- und R-Ibuprofen die Induktion von Apoptose und Zellzyklusarrest über die Aktivierung des Transkriptionsfaktors p53 und dessen Zielgene vermitteln. Verwendet wurde eine HCT-116 Zelllinien, in der beide p53 Allele durch homologe Rekombination inaktiviert waren (p53-/-). Im Gegensatz zu den HCT-116 p53wt Zellen führte die Behandlung mit S- bzw. R-Ibuprofen in den p53 defizienten Zellen zu keinem signifikanten G1-Zellzyklusblock und auch die Apoptoserate war in diesen Zellen deutlich geringer als in p53wt Zellen. In vivo zeigte eine tägliche i.p. Injektion von 15 mg/kg S- bzw. R-Ibuprofen bei den Mäusen mit p53-/- Tumoren keine signifikante Wachstumshemmung. Im Vergleich dazu wurde die Proliferation der HCT-116 p53wt Tumore durch die R-Ibuprofen Behandlung signifikant gehemmt, und auch die mit S-Ibuprofen behandelten Tieren zeigten deutlich kleinere Tumore als die Gruppe mit den p53 defizienten Tumoren. Nach der Inkubation mit S- bzw. R-Ibuprofen wurde in beiden HCT-116 Zelllinien eine erhöhte Expression des Membranrezeptors p75NTR beobachtet. Außerdem führte die Behandlung mit S- bzw. R-Ibuprofen in den p53wt Zellen, im Gegensatz zu den p53 defizienten Zellen, zu einer vermehrten Bax Expression und einem Anstieg an Bax Protein in der mitochondrialen Fraktion. Schließlich wurde gezeigt, dass die erhöhte p53 Konzentration in den Zellen zumindest teilweise über den p75NTR Rezeptor reguliert wird, da durch die externe Zugabe des Liganden NGF die Ibuprofen induzierte Akkumulation von p53 in den HCT-116 p53wt Zellen wieder aufgehoben wurde. Da in vitro in den HCA-7 Zellen nicht nur die Behandlung mit S-Ibuprofen, sondern auch die mit R-Ibuprofen zu einer Hemmung der PGE2 Synthese führte, wurde im dritten Teil der Arbeit die Wirkung von S- und R-Ibuprofen auf die Expression der PGE-Synthasen und auf die mPGES-1 Aktivität untersucht. Dabei wurden HeLa Zellen verwendet, deren mPGES-1 Expression durch die Zugabe von IL-1ß und TNF-alpha stimmuliert wurde. Nach gleichzeitiger Inkubation mit S- bzw. R-Ibuprofen wurde keine Inhibierung der mPGES-1, mPGES-2 oder cPGES Expression beobachtet. Auch die Enzymaktivität von mPGES-1 wurde weder durch die Inkubation mit S-Ibuprofen noch mit R-Ibuprofen beeinflußt. Die bei der Inkubation mit R-Ibuprofen auftretende Hemmung der PGE2 Synthese konnte in der vorliegenden Arbeit nicht abschließend erklärt werden und bietet somit einen interessanten Ansatzpunkt für weitere Untersuchungen.
LC-MS/MS-Systeme haben sich in den vergangenen Jahren trotz der hohen Anschaffungskosten von mehreren hunderttausend Euro zu einer Standardmethode in analytischen Laboratorien entwickelt und bieten im Vergleich zu herkömmlicher HPLC mit UV- und Fluoreszenz-Detektoren eine höhere Sensitivität und bessere Selektivität. Aufgrund dieser Vorteile war es das Ziel dieser Arbeit, verschiedene LC-MS/MS-Methoden zu entwickeln, mit der Eicosanoide sensitiv und selektiv in verschiedenen biologischen Flüssigkeiten quantifiziert werden können und diese Assays dann in biologischen Fragestellungen einzusetzen. Ein Problem der Eicosanoidanalytik mittels LC-MS/MS besteht in der Strukturähnlichkeit vieler Prostaglandine, die große Schwierigkeiten während einer Methodenentwicklung verursachen. Zwei sehr wichtige Prostaglandine, PGE2 und PGD2, besitzen das gleiche Fragmentspektrum und können nicht über das Selektionsprinzip des Tandem-Massenspektrometers quantifiziert werden, sondern müssen vor der Detektion chromatographisch getrennt werden. Die chromatographischen Bedingungen, die für eine Trennung von PGE2 und PGD2 nötig sind, gehen allerdings mit einem erheblichen Empfindlichkeitsverlust des Massenspektrometers einher. Die beste Empfindlichkeit wurde mit einem neu entwickelten Säulenmaterial, Synergi Hydro-RP, erreicht, was in allen entwickelten Methoden verwendet wurde. Zur Quantifizierung von PGE2 und PGD2 in Mikrodialysaten (max. Volumen 75 µl) musste eine Empfindlichkeit von mindestens 40 pg/ml PGE2 erreicht werden. Nach der Validierung des Systems konnte eine Quantifizierungsgrenze von PGE2 und PGD2 von 25 pg/ml bzw. 50 pg/ml erreicht werden. Bei Formalin-behandelten Ratten stieg PGE2 innerhalb von 15 Minuten auf 380 pg/ml an, womit frühere Ergebnisse, die auf der Verwendung von Immunoassays basieren, bestätigt werden konnten. Für PGD2 konnte kein Unterschied zu Kontroll-Ratten festgestellt werden und ist deswegen im Gegensatz zu PGE2 nicht an der Nozizeption im untersuchten Modell beteiligt. Mit Hilfe des mPGES-1-Enzym-Assays wurde mPGES-1 als ein weiteres Target für die drei COX-Inhibitoren Celecoxib, R- und S-Flurbiprofen identifiziert. Da mit Celecoxib ab einer Konzentration von 100 µM keine weitere Hemmung der mPGES-1 erreicht werden kann, hemmt Celecoxib die mPGES-1 wohl nicht kompetitiv und bindet nicht im katalytischen Zentrum der mPGES-1. Für Rofecoxib und Paracetamol konnte keine Hemmung der mPGES-1 festgestellt werden. Während der Methodenentwicklung wurde ein Störpeak zwischen PGE2 und PGD2 beobachtet, der in den Versuchen immer im gleichen Verhältnis zu PGE2 gebildet wurde. Mit Hilfe eines Fingerprints wurde dieser Störpeak als 5-trans-PGE2 identifiziert, welches ein bisher nicht beschriebenes Nebenprodukt der mPGES-1 zu sein scheint. In der letzten Methode sollte eine Methode entwickelt werden, mit der PGE2, PGD2, PGF2alpha, 6-keto-PGF1alpha und TXB2 in Humanplasma bestimmt werden können. Im direkten Vergleich war die validierte LC-MS/MS-Methode einem handelsüblichen Immunoassay überlegen. Zum einem waren die Standardabweichungen der LC-MS/MS-Methode wesentlich niedriger und zum anderen waren die Basalwerte des Immunoassays unphysiologisch hoch. In einer Studie wurde der Einfluss einer Mutation (-765G -> C) im Promotor der COX-2 auf die COX-2-Aktivität untersucht, für die eine reduzierte COX-2-Expression um den Faktor 2 in Monozyten beschrieben wurde. Zwischen den 2 Gruppen konnten weder in den Prostaglandin-Konzentrationen, der COX-2-mRNA-Expression noch in der COX-2-Protein-Expression statistische Unterschiede festgestellt werden und stehen damit im Widerspruch zu bisherigen Veröffentlichungen. Die entwickelten LC-MS/MS-Methoden ermöglichen eine bessere Sensitivität und schnellere Analysenzeiten im Vergleich zur HPLC und durch ihre gute Sensitivität kann der Einsatz von problematischen Immunoassays vermieden werden. Im Falle der Mikrodialysemethode, wo PGE2 in einer relativ einfachen Matrix bestimmt werden sollte (ACSF), liefert die LC-MS/MS-Methode die gleichen Ergebnisse. Im Falle von Humanplasma ist der Immunoassay allerdings unbrauchbar. Ein weiterer Vorteil der LC-MS/MS-Methoden gegenüber Immunoassays besteht darin, dass mehrere Prostaglandine in einer einzigen Probe bestimmt werden können. Gegenüber GC-MS und GC-MS/MS-Methoden fehlt den LC-MS/MS-Methoden zwar einiges an Empfindlichkeit, allerdings wird hierfür keine Derivatisierung benötigt.
The µ-opioid receptor is the primary target structure of most opioid analgesics and thus responsible for the predominant part of their wanted and unwanted effects. Carriers of the frequent genetic µ-opioid receptor variant N40D (allelic frequency 8.2 - 17 %), coded by the single nucleotide polymorphism A>G at position 118 of the µ-opioid receptor coding gene OPRM1 (OPRM1 118A>G SNP), suffer from a decreased opioid potency and from a higher need of opioid analgesics to reach adequate analgesia. The aim of the present work was to identify the mechanism by which the OPRM1 118A>G SNP decreases the opioid potency and to quantify its effects on the analgesic potency and therapeutic range of opioid analgesics.
To elucidate the consequences of the OPRM1 118A>G SNP for the effects of opioid analgesics, brain regions of healthy homozygous carriers of the OPRM1 118A>G SNP were identified by means of functional magnetic resonace imaging (fMRI), where the variant alters the response to opioid analgesics after painful stimulation. Afterwards, the µ-opioid receptor function was analyzed on a molecular level in post mortem samples of these brain regions. Finally, the consequences of the OPRM1 118A>G SNP for the analgesic and respiratory depressive effects of opioids were quantified in healthy carriers and non-carriers of OPRM1 118A>G SNP by means of experimental pain- and respiratory depression-models.
To identify pain processing brain regions, where the variant alters the response to opioid analgesics after painful stimulation, we investigated the effects of different alfentanil concentration levels (0, 25, 50 and 75 ng/ml) on pain-related brain activation achieved by short pulses (300 msec) of gaseous CO2 (66% v/v) delivered to the nasal mucosa using a 3.0 T magnetic head scanner in 16 non-carriers and nine homozygous carriers of the µ-opioid receptor gene variant OPRM1 118A>G. In brain regions associated with the processing of the sensory dimension of pain (pain intensity), such as the primary and secondary somatosensory cortices and the posterior insular cortex, the activation decreased linearly in relation to alfentanil concentrations, which was significantly less pronounced in OPRM1 118G carriers. In contrast, in brain regions known to process the affective dimension of pain (emotional dimension), such as the parahippocampal gyrus, amygdala and anterior insula, the pain-related activation disappeared already at the lowest alfentanil dose, without genotype differences.
Subsequently, we investigated the µ-opioid receptor-expression ([3H]-DAMGO saturation experiments, OPRM1 mRNA analysis by means of RT-PCR), the µ-opioid receptor affinity ([3H]-DAMGO saturation and competition experiments) and µ-opioid receptor signaling ([35S]- GTPγS binding experiments) in post mortem samples of the human SII-region, as a cortical projection region coding for pain intensity, and lateral thalamus, as an important region for nociceptive transmission. Samples of 22 non-carriers, 21 heterozygous and three homozygous carriers of OPRM1 118A>G SNP were included into the analysis. The receptor expression and receptor affinity of both brain regions did not differ between non-carriers and carriers of the variant N40D. In non-carriers, the µ-opioid receptors of the SII-region activated the receptor bound G-protein more efficiently than those of the thalamus (factor 1.55-2.27). This regional difference was missing in heterozygous (factor 0.78-1.66) and homozygous (factor 0.66-1.15) carriers of the N40D variant indicating a reduced receptor-G-protein-coupling in the SII-region.
Finally, the consequences of the alteration of µ-opioid receptor function in carriers and noncarriers of the genetic variant was investigated using pain- and respiratory depression-models. Therefore, 10 healthy non-carriers, four heterozygous and six homozygous carriers of the µ- opioid receptor variant N40D received an infusion of four different concentrations of alfentanil (0, 33.33, 66.66 and 100 ng/ml). At each concentration level, analgesia was assessed by means of electrically (5 Hz sinus 0 to 20 mA) and chemically (200 ms gaseous CO2 pulses applied to the nasal mucosa) induced pain, and respiratory depression was quantified by means of hypercapnic challenge according to Read and recording of the breathing frequency. The results showed that depending on the used pain model, both heterozygous and homozygous carriers of the variant N40D needed 2 – 4 times higher alfentanil concentrations to achieve the same analgesia as non-carriers. This increase seems to be at least for homozygous carriers unproblematic, because to reach a comparable respiratory depression as non-carriers, they needed 10-12 times higher alfentanil concentrations.
The results of this work demonstrate that the µ-opioid receptor variant N40D causes a regionally limited reduction of the signal transduction efficiency of µ-opioid receptors in brain regions involved in pain processing. Thus, the painful activation of sensory brain regions coding for pain intensity is not sufficiently suppressed by opioid analgesics in carriers of the variant N40D. Due to the insufficient suppression in hetero- and homozygous carriers of the variant N40D, the concentration of opioids has to be increased by a factor 2 - 4, in order to achieve the same analgesia as in non-carriers. At the same time, the respiratory depressive effects are decreased to a greater extent in homozygous carriers of the N40D variant as they need a 10 - 12 times higher opioid concentration to suffer from the same degree of respiratory depression as non-carriers. Due to the increased therapeutic range of opioid analgesics, an increase of the opioid dose seems to be harmless, at least for homozygous carriers of the N40D variant.
Cancer microenvironment is now recognized as a critical regulator of all stages of cancer development. Beside the tumor vasculature and tumor-infiltrating immune cells, other stromal cells such as cancer-associated fibroblasts (CAFs) regulate tumor growth. Fibroblasts are ubiquitous cells in connective tissue, where they shape the extracellular matrix (ECM). Fibroblasts are usually quiescent but get activated when tissue homeostasis is disturbed. Then, activated fibroblasts rebuild the ECM and communicate with local cells to participate in wound repair. These repair properties can go awry when being unchecked, which can lead to fibrosis and subsequently cancer development. CAFs can promote cancer development by fostering tumor cell growth, polarizing immune cells to an immunosuppressive phenotype, and crosslinking collagen to enable tumor cell invasion. Molecular mechanisms of CAF activation, thus, need to be understood to target these cells in tumors. Prostanoid prostaglandin E2 (PGE2) is viewed as a pro-tumor lipid mediator as suggested by studies pharmacologically or genetically targeting the enzymes producing PGE2, such as microsomal PGE synthase-1 (mPGES-1) in tumor models. Similar to CAFs, PGE2 drives tumor cell growth and tumor-associated immune suppression. Therefore, I hypothesized that PGE2 may play a role in CAF activation.
This hypothesis was tested in two mouse models of breast cancer (orthotopic grafting model, and polyoma middle T oncogene transgenic model), besides using isolated mammary gland (MG) fibroblasts in vitro. As expected, given the pro-tumor function of PGE2, knocking out mPGES-1 reduced the growth of oncogene-driven and transplanted mammary tumors. Surprisingly, CAF density was markedly increased when mPGES-1 was depleted. Importantly, despite reduced primary tumor growth, I observed enhanced lung metastasis upon mPGES-1depletion. Using MG-derived fibroblasts in vitro furthermore revealed that treatment with PGE2 reduced a TGFβtriggered CAF-like activation state. Importantly, bioinformatics analysis of a human breast cancer patient dataset revealed a negative correlation of a PGE2 production signature with fibroblast marker genes. In a next step I investigated if the increased CAF infiltrate was connected to the reduced tumor growth upon depletion of PGE2. To unravel this, I first asked through which E prostanoid (EP) receptor PGE2 signals in fibroblasts. MG fibroblasts mainly expressed EP3, and EP3 KO fibroblasts showed a hyper-proliferative and activated phenotype, indicating EP3 as the main PGE2 receptor in MG fibroblasts. Co-injecting of EP3 KO MG fibroblasts and tumor cells in WT mice suppressed tumor growth, whereas co-injection of WT fibroblasts with tumor cell in mPGES-1 KO mice increased tumor growth. These data indicate that PGE2 restricts CAF levels through EP3, which supports tumor growth. Whole transcriptome mRNAsequencing of WT and mPGES-1 KO FACS-sorted CAFs combined with immunohistochemical data suggested a role of p38 mitogen-activated protein kinase (MAPK) in the modulation of fibroblast activation by PGE2.
In summary, I showed in two breast cancer models that mPGES-1 depletion delays breast cancer progression, which is probably driven by the EP3-PGE2 signaling axis in host stroma. PGE2 appears to be a potent anti-fibroblast activation agent in tumors via EP3 and downstream p38 MAPK signaling. This study therefore hits the dogmatic perception of the general pro-tumor nature of PGE2; showing that PGE2 might be a double-edged mediator that can promote tumor growth at the primary site by restricting CAF expansion, which may in turn hinder infiltration of tumor cells to a secondary site.
I-kappaB-Kinase epsilon - ein neues Zielprotein für die Pharmakotherapie bei Schmerz und Entzündung?

Der Transkriptionsfaktor NF-kappaB spielt eine wichtige Rolle bei der Regulation von Immunantworten, Apoptose und Entzündungen sowie bei der Entstehung und Verarbeitung von Schmerzen. Ein pharmakologischer Eingriff in die NF-kappaB-Aktivierungskaskade könnte daher eine Schmerzhemmung bewirken und so Ansätze für die Entwicklung neuer Therapien für pathophysiologische Schmerzen liefern. Die NF-kappaB-Signalübertragungskaskade bietet verschiedene Angriffspunkte für Pharmaka, wobei zurzeit IkappaB Kinasen (IKK) als hoffnungsvolle Zielmoleküle im Fokus der Untersuchungen stehen. Verschiedene IKKs regulieren die Aktivität von NF-kappaB über die Phosphorylierung des inhibitorischen Proteins IkappaB oder über die direkte Phosphorylierung von NF-kappaB. In der vorliegenden Arbeit wurde die Rolle der neu entdeckten IKK epsilon bei der Schmerzentstehung und -verarbeitung sowie deren Eignung als neues Zielmolekül für die Schmerztherapie näher untersucht. Es konnte gezeigt werden, dass IKK epsilon konstitutiv in Geweben der Maus, welche an der Entstehung und Verarbeitung von Schmerzen beteiligt sind, exprimiert ist. Im Rückenmark konnte die Lokalisation von IKK epsilon in den schmerzrelevanten Laminae I und II des Dorsalhorns nachgewiesen werden und auch in den Hinterwurzelganglien (Dorsal Root Ganglia (DRG’s)) war IKK epsilon in kleinen, nozizeptiven Neuronen exprimiert. Nach peripherer entzündlich-nozizeptiver Stimulation mit Formalin oder Zymosan kam es im Lumbalmark und den DRG’s zu einem signifikanten Anstieg der IKK epsilon-Expression sowohl auf mRNA- als auch auf Proteinebene. Diese Beobachtungen machten eine Beteiligung von IKK epsilon an der Prozessierung von Schmerz sehr wahrscheinlich. Um die Rolle von IKK epsilon während der Schmerzentstehung und -verarbeitung besser beurteilen zu können wurde das Verhalten von IKK epsilon defizienten Mäusen in akuten und inflammatorischen Schmerzmodellen charakterisiert. Es konnte gezeigt werden, dass der Knockout von IKK epsilon zu einem signifikant verringerten nozizeptiven Verhalten im Formalintest und einer Hemmung der mechanischen Hyperalgesie nach Zymosaninjektion im Vergleich zu Wildtyp-Mäusen führte. Gleichzeitig konnte kein Unterschied im akut nozizeptiven Verhalten festgestellt werden. Der Knockout von IKK epsilon hatte demnach keine Auswirkung auf den akuten physiologischen Nozizeptorschmerz, zeigte jedoch eine Verbesserung bei pathophysiologischen Schmerzen. Das verringerte nozizeptive Verhalten der IKK epsilon defizienten Mäuse im Formalintest ging mit einer Hemmung der NF-kappaB-Aktivierung im Rückenmark einher. Auch konnte eine verringerte mRNA-Expression der NF-kappaB-abhängigen Gene Cyclooxygenase-2 (COX-2), Matrixmetalloprotease-9 (MMP-9) und induzierbare Stickstoffmonoxid-Synthase (iNOS), die an der Regulation von Entzündungsschmerzen beteiligt sind, im Rückenmark und den DRG’s nachgewiesen werden. Da IKK epsilon bisher hauptsächlich mit der Aktivierung des TypI Interferon-Signalweges in Zusammenhang gebracht wurde, wurde außerdem geprüft, ob es nach Injektion mit Formalin zu einer Aktivierung des Trankriptionsfaktors Interferon-regulierender Faktor (IRF)-3 in Wildtyp-Mäusen kommt, was nicht beobachtet werden konnte. Der Knockout von IKK epsilon scheint demnach seine antinozizeptive Wirkung direkt über eine fehlende Aktivierung von NF-kappaB zu entfalten, wonach IKK epsilon eine bedeutendere Rolle als bisher angenommen bei der Aktivierung von NF-kappaB spielt. Dies konnte durch in vitro Daten untermauert werden. Der Knockdown von IKK epsilon in Makrophagen-Zellkultur mit spezifischer siRNA verhinderte die Phosphorylierung von NF-kappaBp65 am Serinrest 536 nach Stimulation mit LPS. Anhand der vorliegenden Daten lässt sich also schlussfolgern, dass IKK epsilon an der Schmerzentstehung und verarbeitung bei Entzündungen beteiligt zu sein scheint. Eine Hemmung dieser Kinase könnte demnach ein neues, lohnendes Ziel für die Entwicklung neuer Medikamente für die Schmerztherapie sein.
Das orale Antidiabetikum Glibenclamid ist ein potenter Inhibitor des K ATP Kanals. Die Funktion dieses Kanals ist entscheidend für die Ischämische Präkonditionierung, die Myokardschäden unter Sauerstoffmangel vermindern kann. In der vorliegenden Arbeit wurde der Einfluß der Sulfonylharnstoffe Glimepirid und Glibenclamid in Kombination mit Rilmakalim auf die Kontraktilität isolierter Kardiomyozyten des Meerschweinchens mittels digitaler Bildanalyse untersucht. Weiterhin wurde der Einfluß dieser Substanzen auf die Vitalität isolierter Kardiomyozyten des Meerschweinchens unter elektrischer Stimulation durch LDHBestimmungen bestimmt. Rilmakalim verminderte die Kontraktilität der isolierten Kardiomyozyten konzentrations abhängig. Die Sulfonylharnstoffe Glimepirid und Glibenclamid zeigten in den untersuchten Konzentrationen keinen Einfluß auf die kardiomyozytäre Kontraktilität. In Kombination mit Rilmakalim konnte gezeigt werden, daß Glimepirid in den untersuchten Konzentrationen (0,03 µmol -- 9 µmol) den kontraktilitätsmindernden Effekt von Rilmakalim geringer hemmt als Glibenclamid. Ein Einfluß auf die Vitalität der Kardiomyozyten unter elektrischer Stimulation konnte bei keiner Substanz in den untersuchten Konzentration nachgewiesen werden. Die durchgeführten Untersuchungen legen den Schluß nahe, daß der Einfluß der Sulfonylharnstoffe auf kardiale K ATP Kanäle unterschiedlich ausgeprägt ist. Glimepirid scheint im Vergleich zu Glibenclamid in geringerem Ausmaß den sarkolemmalen K ATP Kanal zu blockieren. Dies läßt vermuten, daß auch kardiovaskuläre Nebenwirkungen, die über den Einflußt auf den K ATP Kanal vermittelt werden, unter einer Glimepiridtherapie geringer ausgeprägt sind. Inwieweit diese Annahme zutrifft, muß durch Untersuchungen an humanen Kardiomyozyten und in weiteren klinischen Studien verifiziert werden. Neuere Erkenntnisse über unterschiedliche K ATP Kanaltypen innerhalb des Kardiomyozyten machen es erforderlich, die Wirkung der Sulfonylharnstoffe auf diese Kanalsubtypen näher zu untersuchen.
Valproinsäure (VPA) gehört zu den am häufigsten verordneten Antiepileptika und Mood Stabilisern. Der Histondeacetylase (HDAC)-Inhibitor VPA wurde mit der Stimulierung der Replikation von einigen Viren in Zusammenhang gebracht. Hierzu gehören HCMV, HIV, HHV-6, HHV-8, Masernviren und Polioviren Typ 1. In der vorliegenden Arbeit wurde der Einfluss von therapeutischen VPA-Konzentrationen und strukturell veränderten VPA-Derivaten auf die HCMV-Replikation in humanen Vorhautfibroblasten untersucht. Die Bestimmung der HCMV IEA Expression in Vorhautfibroblasten zeigte, dass eine 24 Stunden lange Vorbehandlung der Zellen mit VPA in Konzentrationen bis 1 mM, welche auch im Blutplasma der mit VPA behandelten Patienten erreicht wird, die HCMV-Infektionsrate erhöht. Es wurden vergleichbare Ergebnisse für das Patienten-Isolat HCMV Hi91 und die Laborstämme AD169 und Towne erhalten. Strukturell modifizierte VPA-Derivate zeigten einen unterschiedlichen Einfluss auf die HCMV-Replikation. Kettenverzweigung in 3-Position (3-Propylhexansäure und 3-Propylheptansäure) oder Verzweigung in der Seitenkette (2-Ethyl-4-Methylpentansäure) zeigten keinen Einfluss auf die HCMV-Replikation. Verlängerung der gesättigten aliphatischen Kette (2-Propylhexansäure) und Einfügen einer Dreifachbindung in Position 4 (2-Pentyl-Pent-4-insäure und 2-Hexyl-Pent-4-insäure) führten zu einer erhöhten HCMV IEA- und LA-Expression. Die Verbindungen mit einer Dreifachbindung in Position 4 zeigten die stärkste Stimulation. Diese Ergebnisse zeigen, dass die Stimulation der HCMV-Replikation durch die VPA-Derivate sehr stark strukturabhängig ist. Histonacetylierung wurde als Marker für eine HDAC-Hemmung untersucht. VPA-Derivate, die die HCMV-Replikation nicht stimulierten, zeigten keinen Einfluss auf die Acetylierung des Histons H4. VPA und VPA-Derivate, die die Virusreplikation stimulierten, führten zur Akkumulation des acetylierten Histons H4. Die stärkste Akkumulation wurde, analog der HCMV-Replikation, unter dem Einfluss der Verbindungen mit einer Dreifachbindung in Position 4 beobachtet. Die Ergebnisse zeigen eine Korrelation der Akkumulation des acetylierten Histons H4 als Zeichen einer HDAC-Hemmung und der Expression der HCMV IEA und LA. Dies deutet darauf hin, dass die VPA-induziere HDAC-Hemmung zu der erhöhten HCMV-Antigen-Expression beiträgt.
Development of treatment strategies of chronic inflammatory disorders relies on on-going progress in drug discovery approaches and related molecular biologics. This study presents a gene reporter-based approach of phenotypic screening for anti-inflammatory compounds in the context of rheumatoid arthritis (RA).
CEBPD gene, used as the target gene for the screening readout, encodes CCAAT/enhancer binding protein delta (C/EBPδ) transcription factor (TF). Structural and regulatory characteristics of CEBPD gene as well as function of C/EBPδ TF in the context of inflammation satisfied assay requirements. C/EBPδ TF acts as a key regula-tor of inflammatory gene transcription in macrophages (Mϕ) and is observed to con-tribute to disease development in both a rodent model of RA and RA patient biopsies.
Despite well-described pro-inflammatory effects of C/EBPδ TF, it functions as a cell context-specific signal integrator showing also an anti-inflammatory activity. Conse-quently, both activation and inhibition of CEBPD alike may display a desired anti-inflammatory effect. The aim of this study was to develop a high-throughput screening assay for
CEBPD-modulating compounds and confirm hit compounds’ anti-inflammatory effects via gene expression analysis.
Generation and characterization of a multi-gene-reporter cassette 1.0 encoding enzy-matic secreted alkaline phosphatase (SEAP) gene reporter was a priority during the assay development. Chemiluminescent SEAP assay demonstrating high assay sensitivi-ty, broad linear range, high reproducibility and repeatability was chosen to monitor activity of the defined CEBPD promoter (CEBPD::SEAP). PMA-differentiated and M1-polarized THP-1-derived Mϕ stably expressing multi-gene-reporter cassette 1.0 were used as the assay’s cellular system. mRNA expression of both reporter CEBPD::SEAP and endogenous CEBPD mirrored each other in response to a LPS and IFN-g-triggered inflammatory stimulus (M1 treatment), even though the defined CEBPD promoter re-gion, utilized in the assay, contained only the most proximal and known regulatory se-quences. SEAP chemiluminescence in the reporter cells´ supernatant reliably correlat-ed with the M1 treatment-induced CEBPD::SEAP gene expression. The final screening protocol was developed for semi-automatic screening in the 384-well format.
In total, 2054 compounds from LOPAC®1280 and ENZO®774 libraries were screened twice
using the enzymatic SEAP readout with subsequent analysis of 18 selected compounds: nine with the highest and nine with the lowest signals, further characterized by qPCR. Gene expression levels of endogenous CEBPD, CEBPD::SEAP reporter as well as, IL-6,
IL-1β, and CCL2 as inflammatory markers were quantified. qPCR assays failed to corre-late to SEAP readout in 15 compounds within three standard deviations (SDs) from sol-vent control: nine low signal and six high signal compounds. Demonstrating both assay sensitivity and specificity, a correlation between qPCR gene expression and SEAP readout was observed for three hit compounds with signals above three SDs: BET inhib-itors (BETi) GSK 1210151A and Ro 11-1464 as well as an HDAC inhibitor (HDACi) vori-nostat. The control compound trichostatin A (TSA) that reproducibly upregulated SEAP readout is also an HDAC inhibitor with a similar structure to vorinostat and was there-fore included in the anti-inflammatory phenotype analysis.
The observed suppression of IL-6, IL-1ß, and CCL2 gene expression by hit compounds suggested their anti-inflammatory effect in THP-1 reporter Mϕ. mRNA expression of
IL-6 and CCL2 was suppressed by HDACi and BETi at both 4 and 24 hours, while BETi reduced IL-1β mRNA expression 24 hour time point. BETi significantly upregulated gene expression of both reporter CEBPD::SEAP and endogenous CEBPD, 4 hours after M1 treatment. At the same time point, HDACi completely abolished the mRNA expres-sion of the endogenous CEBPD, while simultaneously upregulating mRNA expression of the reporter CEBPD::SEAP. The use of the most proximal 300 base pairs region of en-dogenous CEBPD promoter, making the upstream regulatory elements unavailable in the assay, may account for differential expression levels of SEAP and C/EBPδ TF. This observation corroborated the need to include a longer and more extensive CEBPD´s gene regulatory area. Thus, an improved multi-gene-reporter cassette 2.0 was gener-ated to be used on the basis of a bacterial artificial chromosome (BAC) covering CE-BPD´s genomic area of about 200,000 base pairs.
The generated screening assay is flexible, reliable, and sensitive displaying potential for drug discovery and drug repurposing. The pharmacological modulation of CEBPD gene expression, first reported for GSK 1210151A, Ro 11-1464, and vorinostat, contrib-utes to the understanding of inflammatory responses in Mϕ and may have RA thera-peutic applications.
The scope of this thesis is to elaborate on the use cases of the EEG in pain research. It has been submitted as a cumulative dissertation, meaning that the main part of this thesis has been previously published in international peer-reviewed journals. The first part of this thesis begins with an introduction which describes the general methodoligcal considerations and theoretical background information that is needed to perform pain research using the EEG. Then, I will give a summary of the results of all three studies and the subsequently published manuscripts. The discussion will give an outlook on two ongoing projects and elaborate how the methodology that has been compiled throughout my time as a PhD student can be further applied to scientific problems in pain research. I will conclude with the possibilities and the limitations of the EEG in pain research. The second part of this thesis consists of three publications that cover three individual studies, of which I am the lead/first author. These publications describe different use cases for the EEG in pain research. The first publication lays out the methodological backbone of this thesis, analyzing the exact EEG parameters that are needed to achieve the results in the following projects. Then, I present two additional studies. The first study describes the usefulness of pain-related evoked signatures after standardized noxious stimulation in the EEG in patients undergoing general anesthesia. The second study outlines differences in the pain processing of elite endurance athletes versus a normally active control group. Furthermore, it outlines how the function of the endogenous pain modulatory system can be measured in the EEG using CPM. All studys are discussed individually as per the journal guidelines.
Slack (sequence like a Ca2+ -activated K + channel; also termed Slo2.2, Kcnt1, or KNa 1.1) is a Na+ -activated K + channel that is highly expressed in the peripheral and central nervous system. Previous studies have shown that Slack is enriched in the isolectin B4binding, non-peptidergic subpopulation of C-fiber sensory neurons and that Slack controls the sensory input in neuropathic pain. Recent single-cell RNA-sequencing studies suggested that Slack is highly co-expressed with transient receptor potential (TRP) ankyrin 1 (TRPA1) in sensory neurons. By using in situ hybridization and immunostaining we confirmed that Slack is highly co-localized with TRPA1 in sensory neurons, but only to a minor extent with TRP vanilloid 1. Mice lacking Slack globally or conditionally in sensory neurons (SNS-Slack─/─ ), but not mice lacking Slack conditionally in neurons of the spinal dorsal horn (Lbx1-Slack─/─ ), displayed increased pain behavior after intraplantar injection of the TRPA1 activator allyl isothiocyanate. Patch-clamp recordings with cultured primary neurons and in a HEK-293 cell line transfected with TRPA1 and Slack revealed that Slack-dependent K + currents are modulated in a TRPA1-dependent manner. Taken together, these findings highlight Slack as a modulator of TRPA1-mediated activation of sensory neurons.
Furthermore, we investigated the contribution of Slack in the spinal dorsal horn to pain processing. Lbx1-Slack ─/─ mice demonstrated normal basal pain sensitivity and Complete Freund’s Adjuvant-induced inflammatory pain. Interestingly, we observed a significantly increased spared nerve injury (SNI)-induced neuropathic pain hypersensitivity in Lbx1-Slack ─/─ mutants compared to control littermates. Moreover, we tested the effects of pharmacological Slack activation in the SNI model. Systemic and intrathecal, but not intraplantar administration of the Slack opener loxapine significantly alleviated SNI-induced hypersensitivity in control mice, but only slightly in Lbx1Slack ─/─ mice, further supporting the inhibitory function of Slack in spinal dorsal horn neurons in neuropathic pain processing.
Altogether, our data suggest that Slack in sensory neurons controls TRPA1-induced pain, whereas Slack in spinal dorsal horn neurons inhibits peripheral nerve injury induced neuropathic pain. These data provide further insights into the molecular mechanisms of pain sensation.
