Vaccination represents one of the fundamentals in the fight against SARS-CoV-2. Myocarditis has been reported as a rare but possible adverse consequence of different vaccines, and its clinical presentation can range from mild symptoms to acute heart failure. We report a case of a 29-year-old man who presented with fever and retrosternal pain after receiving SARS-CoV-2 vaccine. Cardiac magnetic resonance imaging and laboratory data revealed typical findings of acute myocarditis.
Aortic valve (AV) and root replacement with composite graft and re-implantation of coronary arteries described first by Bentall and de Bono in 1968, is considered as a standard operation for treatment of different pathologies of the AV and aortic root. In centres where aortic valve and root repair techniques and Ross operation are well established, generally severely diseased patients remain indicated for this procedure. The aim of this study was to evaluate the early and long-term outcomes after Bentall-De Bono (BD) procedures in high-risk population with complex pathologies and multiple comorbidities.
Between 2005 and 2018, a total of 273 consecutive patients (median age 66 years; 23 % female) underwent AV and root replacement with composite-graft in so called button technique. We divided our population in the following groups: 1. acute type A aortic dissection group (ATAAD) (n = 48), 2. endocarditis group (n = 99) and 3. all other pathologies group (n = 126). The surgery has been per- formed emergent/urgent in 131 patients (49 %) and in 109 cases (40%) as a reoperation. Concomitant surgery was required in 97 patients (58%) and 167 pa- tients (61%) received a biological composite-graft.
Follow-up was completed in 96% (10 patients lost to follow-up) with a mean of 8.6 years (range 0.1-15.7 years), counting a total of 1450 patient-years. Thirty- day mortality was 17% (46 patients). The overall estimated survival in 5 and 10 years was 64% ± 3%) and 46% ±4 %). Group comparison showed a significant difference in favour of patient from the dissection group (p = 0.008). Implantation of a biological valve graft was associated with lower survival probability (p < 0.001). There was no significant difference in the freedom of reoperation rate between the groups. The same applies for freedom of postoperative endocarditis, thromboembolic events, and aortic prosthesis dysfunction. According to the uni- variate and multivariate logistic regression analysis primarily postoperative neu- rological dysfunction (OR 5.45), hypertension (OR 4.8) peripheral artery disease (OR 4.4), re-exploration for bleeding (OR 3.37) and postoperative renal replace- ment therapy (OR 3.09) were identified as leading predictors of mortality.
In conclusion, the BD operation can be performed with acceptable short- and long-term results in high-risk patients with complex aortic pathologies in a centre with well-established AV repair and Ross operation program.
Die vorliegende Arbeit thematisiert den Vergleich der Bildqualität von Liegend-Röntgen-Thorax-Aufnahmen von Patienten auf der Intensivstation des Universitätsklinikums Frankfurt unter Verwendung einerseits eines parallelen und andererseits eines virtuellen Streustrahlenrasters (=Bildverarbeitungssoftware). Es wurde untersucht, ob mit dem virtuellen Raster eine mindestens gleichwertige Bildqualität wie mit dem parallelen Raster erreicht und gleichzeitig Strahlendosis eingespart werden kann.
Insgesamt wurden 378 Röntgen-Thorax-Aufnahmen von 126 Patienten, die jeweils einmal mit parallelem Raster, mit virtuellem Raster und mit dem gleichen virtuellen Raster mit Dosisreduktion durchgeführt wurden, in die Studie eingeschlossen. Das virtuelle Raster ahmt das parallele Raster in der Streustrahlenreduktion nach. Das Übergewicht der Patienten als Einschlusskriterium der Studie rechtfertigte den Einsatz des parallelen Rasters. Jeder Patient wurde nur nach klinischer Indikationsstellung geröntgt, sodass der zeitliche Abstand zwischen zwei Röntgen-Thorax-Aufnahmen unterschiedlicher Aufnahmetechniken desselben Patienten variierte. Für alle Röntgen-Thorax-Aufnahmen wurde derselbe indirekte Flachdetektor verwendet. Die Röhrenspannung betrug konstant 125 kV, das Strom-Zeit-Produkt 1,4 mAs (für das parallele und virtuelle Raster) bzw. 1,0 mAs (für das virtuelle Raster mit Dosisreduktion). Für jeden Röntgen-Thorax wurde das Dosisflächenprodukt bestimmt. Vier Radiologen evaluierten die Bildqualität hinsichtlich sechs Kriterien (Lungenparenchym, Weichteile, thorakale Wirbelsäule, Fremdkörper, Pathologien und Gesamtqualität) anhand einer 9-Punkte-Skala. Der Friedman-Test (p < 0,05: signifikant) wurde angewendet. Die Übereinstimmung der Radiologen wurde über Intraklassenkorrelationskoeffizienten berechnet.
Das virtuelle Raster ohne/mit Dosisreduktion wurde insgesamt von allen vier Radiologen für die Weichteile, die thorakale Wirbelsäule, die Fremdkörper und die Gesamtbildqualität signifikant besser bewertet als das parallele Streustrahlenraster (p ≤ 0,018).
Für das Lungenparenchym und die Pathologien resultierten sowohl signifikante als auch nicht-signifikante Ergebnisse, wobei bei signifikanten Ergebnissen ebenfalls das virtuelle Raster ohne/mit Dosisreduktion besser bewertet wurde als das parallele Streustrahlenraster (p ≤ 0,002). Einzige Ausnahme stellten die Evaluationen der Bildqualität bez. des Lungenparenchyms eines Radiologen dar, der das virtuelle Raster ohne/mit Dosisreduktion signifikant schlechter bewertete als das parallele Raster (p < 0,0001). Insgesamt wurde das virtuelle Raster mit Dosisreduktion für die folgenden Kriterien am besten in absteigender Reihenfolge im Vergleich zum parallelen Raster bewertet: Fremdkörper, thorakale Wirbelsäule, Weichteile, Gesamtbildqualität, Pathologien und Lungenparenchym. Die Übereinstimmung der vier Radiologen in ihren Bildqualitätsbewertungen war maximal gering. Mit dem virtuellen Raster wurde im Durchschnitt etwa 28,7% des Dosisflächenprodukts im Vergleich zum parallelen Streustrahlenraster eingespart (p < 0,0001).
Bisher haben nur vier Studien Streustrahlenreduktionssoftwares an Liegend-Röntgen-Thorax-Aufnahmen untersucht, davon zwei an lebenden Menschen. Limitationen der vorliegenden Studie sind die Subjektivität der Bewertungen der Radiologen, die mögliche Identifizierung der Röntgen-Thorax-Aufnahmen, die mit dem parallelen Streustrahlenraster als gängige Aufnahmetechnik in der Radiologie des Universitätsklinikums Frankfurt durchgeführt wurden, die Konstanz der Expositionsparameter unabhängig des BMI der Patienten und die eingeschränkte Vergleichbarkeit der Röntgen-Thorax-Aufnahmen desselben Patienten aufgrund von Veränderungen der Pathologien, Fremdkörper, etc. bei (großem) zeitlichem Abstand zwischen den Röntgen-Thorax-Aufnahmen.
Das virtuelle Raster erzielte teils eine gleichwertige, teils eine bessere Bildqualität wie/als das parallele Raster bei gleichzeitiger Dosisreduktion von 28,7% und kann es somit bei Liegend-Röntgen-Thorax-Aufnahmen ersetzen. Weitere Studien sollten den Einsatz des virtuellen Rasters bei Röntgenaufnahmen des Thorax (stehend und liegend) und anderer Körperpartien im Hinblick auf die Bildqualität, (höhere) Dosiseinsparungen und den Workflow untersuchen.
Cellular response of human osteoblasts to different presentations of deproteinized bovine bone

Objectives: This study evaluated the cellular response of primary osteoblasts exposed to two different presentations of a low-temperature non-sintered deproteinized bovine bone matrix (DBBM). Materials and methods: Six different baths of a commercially available DBBM block (Bonefill® Porous Block) and one of DBBM granule (Bonefill® Porous) were evaluated to identify the mineral structure and organic or cellular remnants. Samples of the same baths were processed in TRIZOL for RNA extraction and quantification. For the immunologic cell reaction assay, primary human osteoblasts (pOB) were exposed to DBMM block (pOB + B) or granules (pOB + G), or none (control) for 1, 3, or 7 days of cell cultivation. Expression of proinflammatory cytokines by pOB was evaluated by crosslinked ELISA assay. In addition, total DNA amount, as well as cell viability via LDH evaluation, was assessed. Results: Organic remnants were present in DBBM blocks; 45.55% (±7.12) of osteocytes lacunae presented cellular remnants in blocks compared to 17.31% (±1.31) in granules. In three of five batches of blocks, it was possible to isolate bovine RNA. The highest concentration of TGF-β1 was found in supernatants of pOB + G on day 7 (218.85 ± 234.62 pg/mL) (p < 0.05), whereas pOB + B presented the lowest amount of TGF-β1 secretion at the end of evaluation (30.22 ± 14.94 pg/mL, p < 0.05). For IL-6 and OPG, there was no statistical difference between groups, while pOB + G induced more IL-8 secretion than the control (3.03 ± 3.38 ng/mL, p < 0.05). Considering the kinetics of cytokine release during the study period, all groups presented a similar pattern of cytokines, estimated as an increasing concentration for IL-6, IL-8, and OPG during cultivation. Adherent cells were observed on both material surfaces on day 7, according to H&E and OPN staining. Conclusion: Neither tested material induced a pronounced inflammatory response upon osteoblast cultivation. However, further studies are needed to elucidate the potential influence of organic remnants in bone substitute materials on the regeneration process.

The development of epilepsy (epileptogenesis) involves a complex interplay of neuronal and immune processes. Here, we present a first-of-its-kind mathematical model to better understand the relationships among these processes. Our model describes the interaction between neuroinflammation, blood-brain barrier disruption, neuronal loss, circuit remodeling, and seizures. Formulated as a system of nonlinear differential equations, the model reproduces the available data from three animal models. The model successfully describes characteristic features of epileptogenesis such as its paradoxically long timescales (up to decades) despite short and transient injuries or the existence of qualitatively different outcomes for varying injury intensity. In line with the concept of degeneracy, our simulations reveal multiple routes toward epilepsy with neuronal loss as a sufficient but non-necessary component. Finally, we show that our model allows for in silico predictions of therapeutic strategies, revealing injury-specific therapeutic targets and optimal time windows for intervention.
Ziel: Ziel dieser Studie war es die Verweildauerraten, sowie die komplikationslosen Verweil-dauerraten von rein zahn, rein implantat und kombiniert zahn-implantatgetragenen Galvano-Konusprothesen auf keramischen Primärteilen im Oberkiefer zu untersuchen. Zusätzlich wurden durchgeführte Reparaturen, sowie die Patientenzufriedenheit ausgewertet.
Methode: 83 Patienten, welche im Zeitraum von 1999-2012 am ZZMK eine Galvano-Konusprothese im Oberkiefer erhalten haben wurden retrospektiv nachuntersucht. Die Patientenzufriedenheit wurde anhand eines Fragebogens erhoben, welcher mittels 22 Fragen die Bereiche „Ästhetik“, „Prothesenhalt-bzw. funktion“ und die „Reinigungsmöglichkeit der Prothese“ aus Patientensicht evaluiert. Als Zielereignis der Kaplan-Meier Verweildaueranalyse wurde die erste Reparatur, das Versagen der Prothese, sowie der Pfeiler- bzw. der Implantatverlust definiert. Reparatur- und Nachsorgemaßnahmen wurden deskriptiv erhoben.
Ergebnisse: Es konnten 83 Prothesen und 477 Pfeiler nachuntersucht werden. Der mittlere Beobachtungszeitraum betrug 3,9 Jahre. Die 3- ,5- und 10- Jahres Verweildauerraten der Prothesen lagen bei 98,2%, 95,5% bzw. 70,7%. Die konstruktionsbezogene Pfeilerüberlebensrate bis zum ersten Pfeilerzahnverlust betrug 98,2% nach 3 Jahren, 92,9% nach 5 Jahren und 29,2% nach 10 Jahren. Die häufigste Ursache eines Versagens war der Mehrfachverlust von Pfeilern (n=5). Eine Prothese versagte aufgrund eines Gerüstbruchs (n=1). Es konnte keine signifikanten Unterschiede in der Verweildauerrate bis zum Prothesenverlust, sowie in der komplikationslosen Verweildauerrate zwischen rein zahn, rein implantat und kombiniert zahn-implantagetragenen Prothesen festgestellt werden. Zu den häufigsten Reparaturen zählten Erweiterungen nach Pfeilerverlusten, Verblendreparaturen, Dezementierungen der Primärkronen und Frakturen von Prothesenzähnen. Die häufigste Nachsorgemaßnahme war die Druckstellenentfernung. Die Patientenzufriedenheit mit der Galvano-Konusprothese erwies sich als sehr hoch. Die Ästhetik, der Prothesenhalt und die Reinigungsmöglichkeit des Zahnersatzes wurden unabhängig der Verankerungsart durchgängig positiv bewertet.
Schlussfolgerung: Die Galvano-Konusprothese hat sich als hochwertiger herausnehmbarer Zahnersatz bewährt, welcher hohe Verweildauerraten aufweist und dessen Reparaturanfälligkeit vergleichbar mit anderen Doppelkronenversorgungen ist.
Previous studies towards reduced oxygen availability have mostly focused on changes in total mRNA expression, neglecting underlying transcriptional and post-transcriptional events. Therefore, we generated a comprehensive overview of hypoxia-induced changes in total mRNA expression, global de novo transcription, and mRNA stability in monocytic THP-1 cells. Since hypoxic episodes often persist for prolonged periods, we further compared the adaptation to acute and chronic hypoxia. While total mRNA changes correlated well with enhanced transcription during short-term hypoxia, mRNA destabilization gained importance under chronic conditions. Reduced mRNA stability not only added to a compensatory attenuation of immune responses, but also, most notably, to the reduction in nuclear-encoded mRNAs associated with various mitochondrial functions. These changes may prevent the futile production of new mitochondria under conditions where mitochondria cannot exert their full metabolic function and are indeed actively removed by mitophagy. The post-transcriptional mode of regulation might further allow for the rapid recovery of mitochondrial capacities upon reoxygenation. Our results provide a comprehensive resource of functional mRNA expression dynamics and underlying transcriptional and post-transcriptional regulatory principles during the adaptation to hypoxia. Furthermore, we uncover that RNA stability regulation controls mitochondrial functions in the context of hypoxia.
Bone-seeking 223Radium-dichloride (223Ra) is an established treatment prolonging survival and reducing morbidity in selected patients with metastatic castration-resistant prostate cancer (mCRPC) with skeletal involvement. Radioligand therapy with 177Lutetium-PSMA-617 (177Lu-PSMA-617) has been increasingly implemented in patients with mCRPC failing conventional treatment options. In this study, the safety and efficacy of 177Lu-PSMA-617 in patients with progressive bone involvement under treatment with 223Ra was assessed. Twenty-eight men (median age 73 years, range 63–89 years) with progressive mCRPC, who started 177Lu-PSMA-617 within 8 weeks after the last 223Ra administration, received a median of 4 (IQR 3–6) and a total of 120 cycles of 223Ra and a median of 4 (IQR 2–7) cycles 177Lu-PSMA-617 with a mean treatment activity of 6.5 ± 1.2 GBq per cycle, reaching a mean cumulative activity of 30.7 ± 23.4 GBq. A PSA response (≥50% PSA decline 12 weeks after the first 177Lu-PSMA-617 cycle) was observed in 18/28 (64.3%) patients and imaging-based partial remission (PR) was observed in 11/28 (39.3%) patients. Median imaging-based progression-free survival (PFS) was 10 (95% CI, 6–14) months and median overall survival (OS) was 18 (95% CI, 14–22) months. Patients with low bone tumor burden (2–20 lesions) had a significantly longer OS (28 vs. 14 months, p < 0.045) compared to patients with a high tumor burden (>20 lesions). Grade ≥ 3 hematological toxicity was observed in six patients after their last treatment cycle with anemia, leukopenia and thrombocytopenia in 5/28 (17.9%), 4/28 (14.3%) and 6/28 (21.4%) patients, respectively. In progressive bone-metastatic mCRPC patients, prompt initiation of 177Lu-PSMA-617 after failing 223Ra is effective with an acceptable toxicity profile.
