Background: The health status, health awareness and health behavior of persons with a migration background often differ from the autochthonous population. Little is known about the proportion of patients with a migration background (PMB) that participate in primary care studies on oral antithrombotic treatment (OAT) in Germany, and whether the quality of their antithrombotic care differs from patients without a migration background. The aim of this paper was to use the results of a cluster-randomized controlled trial (PICANT) to determine the proportion of PMB at different stages of recruitment, and to compare the results in terms of sociodemographic characteristics and antithrombotic treatment.
Methods: This study used screening and baseline data from the PICANT trial on oral anticoagulation management in GP practices. For this analysis, we determined the proportion of PMB during the recruitment period at stage 1 (screening of potentially eligible patients), stage 2 (eligible patients invited to participate in the trial), and stage 3 (assessment of baseline characteristics of patients participating in the PICANT trial). In addition, we compared patients in terms of sociodemographic characteristics and quality of anticoagulant treatment. Statistical analysis comprised descriptive and bivariate analyses.
Results: The proportion of PMB at each recruitment stage declined from 9.1% at stage 1 to 7.9% at stage 2 and 7.3% at stage 3). A lack of German language skills led to the exclusion of half the otherwise eligible PMB. At stages 1 and 3, PMB were younger (stage 1: 70.7 vs. 75.0 years, p<0.001; stage 3: 70.2 vs. 73.5 years, p = 0.013), but did not differ in terms of gender. The quality of their anticoagulant care was comparable (100.0% vs. 99.1% were receiving appropriate OAT, 94.4% vs. 95.7% took phenprocoumon, or warfarin, and the most recent INR measurement of 60.8% vs. 69.3% was within their individual INR range).
Conclusions: In the potentially eligible population and among participants at baseline, the quality of anticoagulant care was high in all groups of patients, which is reassuring. To enable the inclusion of more PMB, future primary care research on OAT in Germany should address how best to overcome language barriers. This will be challenging, particularly because the heterogeneity of PMB means the resulting sample sizes for each specific language group are small.

Introduction: Reliable and cost-effective diagnostics for hepatitis E virus (HEV) infection are necessary. The aim of our study was to investigate which diagnostic test is most accurate to detect HEV infection in immunocompetent and immunosuppressed patients in a real world setting. Patients and Methods: We performed a retrospective analysis of 1165 patients tested for HEV antibodies and HEV PCR at the same time point. Clinical, laboratory and virological data were taken from patient charts. HEV IgA was measured in a subgroup of 185 patients. Results: HEV RNA was detectable in 61 patients (5.2%); most of them (n = 49, 80.3%/n = 43, 70.5%) were HEV IgM+ and IgG+; however, 12 patients (19.6%) were HEV RNA positive/HEV IgM negative and 17 patients (27.8%) were HEV RNA positive/HEV IgG negative. Ten HEV RNA positive patients (16.4%) had neither HEV IgG nor IgM antibodies. Importantly, all of them were immunosuppressed. HEV IgA testing was less sensitive than HEV IgM for HEV diagnosis. Conclusions: HEV infection can be overlooked in patients without HEV specific antibodies. Performing PCR is necessary to diagnose or exclude HEV infection in immunocompromised hosts. In immunocompetent patients, a screening based on HEV antibodies (IgG/IgM) is sufficient.
Human cytomegalovirus (CMV) is a significant cause of morbidity and mortality in patient groups at risk. We have previously shown that the anti-CMV IgG seroprevalence in an urban region of Germany has changed over the last decades. Overall, a decline from 63.7 to 57.25% had been observed between 1988–1997 and 1998–2008 (p < 0,001). Here, we continuously follow the trends to the most recent decade 2009 to 2018. In a retrospective analysis, we determined the seroprevalence of CMV IgG antibodies in our patient cohort, stratified by gender and selected groups at risk (e.g., patients with HIV infection; women of childbearing age). The overall prevalence of anti-CMV IgG non-significantly declined further from 57.25% in 1998–2008 to 56.48% in 2009–2018 (p = 0.881). Looking at gender differences, overall CMV seroprevalence in males declined to 52.82% (from 55.54% in 1998–2008; p = 0.0254), while it non-significantly increased in females to 59.80%. The high seroprevalence in patients with a known HIV infection further increased from 87.46% in 1998–2008 to 92.93% in the current period (p = 0.9999). In women of childbearing age, no significant changes over the last three decades could be observed. The CMV seroprevalence in oncological patients was determined to be 60.64%. Overall, the former significant decline of CMV seroprevalence between the decades 1988–1997 and 1998–2008 in this urban region of Germany slowed down to a non-significant decrease of 0.77% (1998–2008 vs. 2009–2018). This might be an indicator that CMV seroprevalence has reached a plateau.
Die afrikanische Schlafkrankheit (HAT), übertragen durch die Tsetse Fliege und ausgelöst durch den einzelligen Parasit Trypanosoma brucei, wird vor über 100 Jahren entdeckt. Diese unbehandelt immer tödlich verlaufende Erkrankung zählt zu den Neglected Tropical Diseases. Es existiert keine Impfung und die Behandlung ist nebenwirkungsreich. Das weltweite Forschungsaufkommen zu diesem Thema wird von 1900 bis 2016 anhand von Metadaten untersucht, die aus dem Web of Science Core Collection (Clarivate Analytics) extrahiert wurden. Die 5079 Publikationen werden mittels bibliometrischer Parameter ausgewertet. Diese umfassen chronologische Publikationsparameter, Analysen der Länder, Institutionen, Autoren, Fachzeitschriften, Fachbereiche, Kooperationen und Geschlechterparität. Die Analyse zeigt ein aufkommendes Forschungsinteresse der Kolonialmächte Anfang des 20. Jahrhunderts in den okkupierten Koloniegebieten Afrikas. Zu dem Zeitpunkt als der Erreger entdeckt wird, grassiert eine Epidemie der Ost-und Westafrikanischen Schlafkrankheit. Mit Beginn des Ersten Weltkrieges brechen die Publikationszahlen ein. Erst ab den 1970ern steigen die Artikelzahlen kontinuierlich. Die Vernachlässigung der Erkrankung resultiert in einer Epidemie mit geschätzt 300.000 Fällen (1998). In den letzten Jahren verringerte sich die Anzahl der Neuinfektionen erheblich, im Jahr 2016 werden 2184 Fälle gemeldet. Zwischen 1964 und 2013 kommt es zu einer mehr als 11-fachen Steigerung der Publikationen. Ab 1980 nehmen die Zitierungen sprunghaft zu, wahrscheinlich konnten viele neue Erkenntnisse aufgrund der hohen Prävalenz gewonnen werden. Der meistzitierte und produktivste Fachbereich ist die Biochemistry and Molecular Biology. Der Anteil Pharmacology and Pharmacy nimmt kontinuierlich zu. Die Popularität könnte auf 2 Faktoren zurückgeführt werden, den steigenden Bedarf an neuen Medikamenten und den Einfluss der WHO. 2009 wird NECT als Kombinationstherapie zugelassen, 2018 folgt Fexinidazole. Das durchschnittliche Literaturverzeichnis vergrößert sich, ebenso die Forschungsgruppen, denn die durchschnittliche Autorenzahl steigt von 2,5 (1975) auf 8,03 (2014). Die Zahl der internationalen Kooperationsartikel wird innerhalb der letzten 30 Jahre versechsfacht. Die publikationsstärkste Nation ist die USA, gefolgt von
Großbritannien, weiteren westeuropäischen Staaten sowie Kenia und Nigeria. 36% der 25 produktivsten Länder sind afrikanische. Gemessen an ihren ökonomischen Daten ist die Forschungsleistung extrem hoch. Unter den 15 produktivsten Institutionen ist als einziges afrikanisches das International Livestock Research Institut (Nairobi) vertreten. Diese Forschungseinrichtung wird durch Stiftungen wie Wellcome Trust finanziell unterstützt, welche bei mehreren hundert Publikationen als Funding Agency dient. Zwei weitere außeruniversitäre Institutionen zählen zu den produktivsten: Das Center of Infectious Disease Research (USA) sowie das Institute of Tropical Medicine Antwerp (Belgien). Die zehn produktivsten Länderkooperationen finden innereuropäisch oder mit den USA statt, darunter existiert eine europäisch-afrikanische Kollaboration (ITM Antwerp, Ministry for für Public Health (DR Kongo)). Bei Analyse der europäisch-afrikanischen Kooperationen fällt auf, dass diese mit der ehemaligen Kolonialpolitik korrelieren könnten. Die meisten afrikanischen Nationen publizieren in Kooperationen, eine Ausnahme bildet Nigeria. Sie veröffentlichen nur 18,7% der Artikel in Zusammenarbeit. Sie sind das Land mit dem höchsten BIP der ausgewerteten afrikanischen Nationen. Der produktivste Autor ist E. Pays der an der Université Libre de Bruxelles forscht. Der meistzitierte Autor G.A.M. Cross von der Rockefeller University ist Verfasser von zwei der meistzitierten Artikel. Die erfolgreichsten Wissenschaftler stehen eher am Ende ihrer Karriere und fungieren meist als Letztautoren und als Leiter einer Einrichtung. Obwohl in den letzten Jahren eine zunehmende Geschlechterparität auf dem Forschungsgebiet identifiziert werden konnte, ist die Chancengleichheit für Frauen abhängig vom Land. In Brasilien überwiegt als einzige Nation der Anteil weiblicher Autoren, während Japan den geringsten Frauenanteil besitzt. 11,2% der Artikel erscheinen in der Fachzeitschrift Molecular and Biochemical Parasitology und deckt somit die beiden meistzugewiesenen Themenfelder ab. Infektiöse Erkrankungen, die vor allem Drittweltländer mit geringen finanziellen Möglichkeiten betreffen, müssen im Interesse der Weltgemeinschaft weiter intensiv erforscht werden. Um dies zu ermöglichen ist eine Finanzierung und Stärkung der Wissenschaft in den betroffenen Ländern vor Ort nötig.
Körpersymmetrie gilt unter Evolutionswissenschaftlern als Hinweis auf eine stabile Entwicklungsgeschichte von der Eizelle bis hin zum erwachsenen Lebewesen mit daraus resultierender Gesundheit. Symmetrie steht für gute Abwehrmechanismen trotz einflussnehmender Störfaktoren, die allgegenwärtig sind. Symmetrische Körper und Gesichter werden vom anderen Geschlecht bevorzugt und als attraktiver bewertet. Das Gegenstück der Symmetrie bildet die fluktuierende Asymmetrie als Abweichung von perfekter, bilateraler Symmetrie an verschiedenen Stellen des Körpers. Symmetrie kann somit als Fitnessindikator angesehen werden, also als ein Merkmal des erfolgreichen Umwerbens in der Partnerwahl. Dies geschieht typischerweise durch die Entwicklung eines Merkmals oder durch aufwändigeres Verhalten, dessen Ausbildung für ein weniger „fittes“ Individuum zu kostspielig wäre. Die Kernfragen der vorliegenden Studie waren, ob sich diese Anforderung des Fitnessindikators Symmetrie auf die verborgene Struktur des Kehlkopfes übertragen lässt und ob die Asymmetrie des Kehlkopfes mit der Attraktivität der Stimme assoziiert ist. Die fluktuierende Asymmetrie des Larynx wurde bisher noch nicht untersucht. An der Untersuchung nahmen 80 Versuchspersonen im Alter von 20 bis 70 Jahren teil. Hauptzielgrößen waren Larynxasymmetrie und Stimmattraktivität. Die Larynxasymmetrie wurde mittels Videolaryngostroboskopie und anschließender Asymmetriebewertung auf einer 7-stufigen Skala durch Experten erfasst. Die Attraktivität der Stimme wurde auf Basis eines vorgelesenen phonetischen Standardtexts durch Dritte bewertetet. Weitere Variablen waren die Gesichtsasymmetrie, die durch Vermessung von Fotografien bestimmt wurde, und die durch Dritte bewertete Gesichtsattraktivität. Mit Hilfe eines Fragebogens, der abgesehen von Alter, Geschlecht, Anzahl bisheriger Sexualpartner, Erkältungskrankheiten im vorherigen Jahr sowie eingenommener Medikamente und das Rauchverhalten die Beschwerdenliste nach Zerssen (1976) und die Kurzfassung des Voice Handicap Index (VHI-12) enthielt, wurde versucht, die Gesundheit zu registrieren. Darüber hinaus erbat der Fragebogen bei weiblichen Probanden Angaben zum Menstruationszyklus, um die Fertilität zum Messzeitpunkt zu erfassen. Der Erfassung der Körperkonstitution dienten die Messungen von Größe und Gewicht, des Körperfettanteils und der Muskelmasse sowie die Umfangsbestimmung von Hals, Schulter, Brust, Taille, Hüfte und Bizeps. Das 2D:4D-Längenverhältnis wurde mit Hilfe einer Fotokopie der palmaren Handseite als Indikator für pränatales Testosteron ermittelt. Der zentrale Zusammenhang zwischen Stimmattraktivität und Larynxasymmetrie bestätigte sich nur für Männer. Für diese bestand außerdem eine positive Relation der Stimmattraktivität mit dem Schulter-Hüft-Verhältnis und dem Verhältnis von Brust zu Taille, d. h. Stimmen von Männern mit verhältnismäßig breiter Schulter und breiter Brust wurden attraktiver bewertet. Umgekehrt galt: Je niedriger der BMI, Körperfettanteil, Halsumfang, das Alter und die Anzahl eingenommener rezeptpflichtiger Medikamente, umso höher war die Attraktivitätsbewertung ihrer Stimme. Weibliche Stimmen wurden umso attraktiver bewertet, je jünger die Frauen waren, je größer das Brust-Taille- Verhältnis, je niedriger das Taille Hüft-Verhältnis, je geringen der Halsumfang, je niedriger der Stimmstörungsindex (VHI-12) und je weniger Zigaretten sie geraucht haben. Erwartungsgemäß korrelierte die Larynxasymmetrie bei Männern signifikant negativ mit der Gesichtsattraktivität sowie beinahe signifikant mit dem Schulter-Hüft-Verhältnis. Männliche Larynges wurden außerdem asymmetrischer bewertet, je älter die Person, je höher der BMI, der Körperfettanteil, der Halsumfang und die Anzahl eingenommener Medikamente war. Für Frauen einzig (positiv) signifikant wurde der Zusammenhang zwischen Larynxasymmetrie und Nikotinabusus. Die zusätzlich erfolgten Regressionsanalysen erzielten ein signifikantes Ergebnis für den Zusammenhang zwischen der Stimmattraktivität mit der Muskelmasse für beide Geschlechter, außerdem gelang es, einen signifikanten Zusammenhang zwischen der Larynxasymmetrie und der SHR bei Männern darzustellen. Die (negativen) Korrelationen zwischen der Larynxasymmetrie für Männer mit Stimm- und Gesichtsattraktivität deuten darauf hin, dass Larynxasymmetrie als Fitnessindikator dienen kann. Darüber hinaus lieferte die vorliegende Studie keine einfach interpretierbaren Ergebisse; häufig waren sie geschlechtsspezifisch. Die Dissertation unterstrich damit die Komplexität der Zusammenhänge und wies auf den Stellenwert von Geschlechterunterschieden hin. Weiterführende Untersuchungen sind nötig, um die Ergebnisse zu bestätigen und um den Stellenwert der Larynxasymmetrie für Frauen zu klären.
Der Plexus tympanicus ist ein komplex aufgebautes Nervengeflecht in der Mukosa des Mittelohrs. Aus anatomischen Studien ist bekannt, dass zuführende und wegführende Verbindungen unterschiedlichen Hirnnerven und sympathischen Bahnen angehören. Insbesondere werden parasympathische und sympathische Innervationssysteme beschrieben und damit stellt der Plexus tympanicus einen Plexus des vegetativen Nervensystems dar.
Bisher fehlen detaillierte Analysen über die Chemoarchitektur dieses Plexus. In der vorliegenden Studie soll das Vorhandensein unterschiedlicher Neurotransmitter und -peptide untersucht werden, um Vorstellungen über die Funktion dieses komplexen Geflechts zu entwickeln.
Es wurden immuncytochemische Färbungen an sechs Parallelserien von Kryostatschnitten durchgeführt. Dabei wurden Primärantikörper benutzt, die gegen Cholinacetyltransferase (ChAT), Dopamin-β-Hydroxylase (DBH), Substanz P (SP), Vasoaktives intestinales Peptid (VIP) und Neuropeptid Y (NPY) gerichtet waren. Dadurch konnten sympathische Nervenfasern durch den Nachweis von DBH als Leitenzym für die Noradrenalinsynthese analysiert werden; parasympathische Strukturen konnten durch Anti-ChAT-AK, das Leitenzym für die Acetylcholinsynthese, differenziert werden.
Alle genannten Neurotransmitter und -peptide konnten in den Mittelohrschnitten nachgewiesen werden. Dabei wurden sie in folgenden Lokalisationen gefunden: VIP wurde vor allem in perikapillären Boutons und Gefäßwänden im gesamten Ohrbereich sowie basal im Drüsenbereich des Meatus acusticus externus nachgewiesen. In der Mittelohrschleimhaut war VIP weit verbreitet und gerade im Bereich des Promotoriums waren einzelne Zellen intensiv angefärbt, die Zeichen sekretorischer Aktivität trugen. SP wurde vor allem in netzartigen um Gefäße gelagerten Fasern und in beaded Nervenfasern in der Mittelohrschleimhaut gefunden. Auch im Bereich der Drüsen, vor allem an Talgdrüsen des äußeren Gehörgangs, wurde SP nachgewiesen. NPY-IR zeigte sich in Geflechten um große Gefäße, an motorischen Endplatten der benachbarten Muskulatur, in der Mittelohrschleimhaut, in Nervenstämmen, Ganglien des Mittelohrbereichs und weniger dicht an Drüsen. ChAT-ir Strukturen sind direkt auf dem Knochen aufliegend in der Mittelohrschleimhaut, gefäßbegleitend an motorischen Endplatten und basal an Drüsenzellen vorhanden. ChAT-ir Nervenzellperikaryen wurden in großer Zahl in Ganglien gefunden, außerdem waren die Nerven allgemein leicht positiv. DBH-ir Strukturen wurden zwischen den Drüsen, in den Gefäßwänden der Arterien im Mittelohrbereich und in Nervenstämmen nachgewiesen. Einige DBH-ir Nervenzellperikaryen befanden sich in den Ganglien innerhalb des Mittelohrbereichs und der zuführenden Hirnnerven. Auch in der Mittelohrschleimhaut wurden Perikaryen, teilweise ganglienartig organisiert, gefunden. In verschiedenen Strukturen im Innenohrbereich konnten alle Neurotransmitter und -peptide in unterschiedlich starker Tingierung nachgewiesen werden.
Anhand des Verteilungsmuster lassen sich Kolokalisationen der Neuropeptide mit noradrenergen und cholinergen Neuronen vermuten, die bereits in anderen Studien für verschiedene Komponenten des vegetativen Nervensystems beschrieben wurden. Anhand der vorliegenden Analysen wurden Lokalisationsübereinstimmungen von ChAT und VIP, ChAT und SP und DBH und NPY gefunden.
Diese Studie soll als Grundlage für weitere Untersuchungen dienen. Insbesondere für das Verständnis von Funktionen und Pathologien des Nervengeflechts bedarf es weiterer Forschung. Die vorliegende Arbeit weist eindeutig nach, dass der Plexus tympanicus ein integratives System darstellt, das im Gegensatz zu früheren Vorstellungen einer reinen Durchgangsorganisation alle Voraussetzungen für ein Kontrollsystem erfüllt.
Background: The newly introduced German pediatric screening examination at the end of the third year of life (U7a) incorporates visual function testing in particular; there is no ophthalmic screening during childhood in Germany. The purpose of this study is to investigate the relationship between participation in U7a and visual function at the preschool health examination (PHE) in the sixth year of life. Methods: This study evaluated PHE data from school enrollment years 2009/2010 to 2014/2015 of Rhineland-Palatinate, Germany. Visual acuity (VA) at PHE was assessed with Rodenstock visual acuity test device (tumbling E) wearing glasses if present. The relationship between participation in U7a and VA <0.7 at PHE was calculated for reduced monocular and binocular VA using multiple logistic regression adjusted for potential confounders. Results: Data from 189,704 children (91,041 girls) in 35 out of 36 districts were included. The first children to participate in U7a were enrolled in 2011/2012 school year. In total, 90,339 children (47.6%) had U7a before PHE, while 99,365 (52.4%) had not. VA <0.7 in at least one eye was measured at PHE in 8429 (4.4%) children, and in both eyes in 4345 (2.3%) children. Participation in U7a was not associated with VA <0.7 at PHE (odds ratio 0.99; 95% confidence interval: 0.94–1.04). Conclusions: The proportion of children with VA <0.7 at PHE was high. No beneficial effect of newly introduced German U7a pediatric screening examination was found for reduced VA at PHE.

URPOSE: Today, the majority of medical graduates in countries such as the UK, the US or Germany are female. This poses a major problem for workforce planning especially in urology. We here use first the first time the previously established Brüggmann Groneberg (BG) index to assess if female academic career options advance in urology.
METHODS: Different operating parameters (student population, urology specialist population, urology chair female:male (f:m) ratio) were collected from the Federal Office of Statistics, the Federal Chamber of Physicians and the medical faculties of 36 German universities. Four time points were monitored (2000, 2005, 2010 and 2015). From these data, female to male (f:m) ratios and the recently established career advancement (BG) index have been calculated.
RESULTS: The German hospital urology specialists' f:m ratios were 0.257 (499 female vs. 1944 male) for 2015, 0.195 for 2010, 0.133 for 2005 and 0.12 for 2000. The career advancement (BG) index was 0.0007 for 2000, 0,0005 for 2005, 0.094 for 2010 and 0.073 for 2015. The decrease from 2010 to 2015 was due to an increase in the f:m ratio of hospital urologists and female medical students.
CONCLUSION: The BG index clearly illustrated that there is an urgent need for special academic career funding programs to counteract gender problems in urology. The BG index has been shown to be an excellent tool to assess female academic career options and will be very helpful to assess and document positive or negative changes in the next decades.
In the absence of an active prophylactic vaccine against HIV-1, passively administered, broadly neutralizing antibodies (bnAbs) identified in some chronically infected persons were shown to prevent HIV-1 infection in animal models. However, passive administration of bnAbs may not be suited to prevent sexual HIV-1 transmission in high-risk cohorts, as a continuous high level of active bnAbs may be difficult to achieve at the primary site of sexual transmission, the human vagina with its acidic pH. Therefore, we used Lactobacillus, a natural commensal in the healthy vaginal microbiome, to express bn nanobodies (VHH) against HIV-1 that we reported previously. After demonstrating that recombinant VHHA6 expressed in E. coli was able to protect humanized mice from mucosal infection by HIV-1Bal, we expressed VHHA6 in a soluble or in a cell-wall-anchored form in Lactobacillus rhamnosus DSM14870. This strain is already clinically applied for treatment of bacterial vaginosis. Both forms of VHHA6 neutralized a set of primary epidemiologically relevant HIV-1 strains in vitro. Furthermore, VHHA6 was still active at an acidic pH. Thus, lactobacilli expressing bn VHH potentially represent an attractive vector for the passive immunization of women in cohorts at high risk of HIV-1 transmission.
The aim of this observational study was to follow-up patients with bedtime basal insulin (NPH insulin) added to metformin. In 285 patients with type 2 diabetes, a therapy with bedtime basal insulin added to metformin was started due to failure to achieve a glycaemic goal. Up until July 2019, 272 patients (95.4%) were followed-up (59.5 y, 92.6 kg, diabetes duration 6.6 y, HbA1c 8.4%/68.6 mmol/mol). HbA1c decreased by −1.2% and bodyweight by −1.7 kg after a duration of 31.7 ± 29.1 (range 2–133) months. Severe hypoglycaemia did not occur. In 144/272 patients (52.9%), the therapeutic goal for HbA1c was achieved over 32.7 months. In 69/272 patients (25.4%), the HbA1c target was achieved over 25.0 months (afterwards, therapy with basal insulin was discontinued because HbA1c was under target). In 36/272 patients (13.2%), the HbA1c goal was achieved until the submission of this manuscript (mean duration of treatment 57.4 ± 28.2 (range 13–121) months). Over 90% of patients with type 2 diabetes and failure of metformin reached their HbA1c goal with additional basal insulin at bedtime over several years in association with a reduction of bodyweight and without any event of severe hypoglycaemia.
Background: Previous studies have demonstrated that CF (Cystic Fibrosis) prognosis is dependent of three major parameters: FEV1 (Forced Expiratory Pressure in one second), BMI (Body Mass Index) and need of intravenous antibiotic therapy. The CF centres of Frankfurt, Germany, and Moscow, Russia, care for cystic fibrosis patients. We decided to investigate and compare both centers from 1990 to 2015. No comparable study has been published so far.
Method: German patient data was collected from the national cystic fibrosis database “Muko.web”. Missing values were extracted from the Hospital Information System. Russian patient data were taken directly from the medical records in Moscow. In a descriptive statistical analysis with Bias and R Studio the values were compared.
Result: A total of 428 patients from Moscow (217 male, 211 female; 348 (81,3%) were P. aeruginosa positive) and 159 patients from Frankfurt (92 male, 67 female; 137 (86,2%) with P. aeruginosa positive) were compared with regard to P. aeruginosa positivity, BMI, FEV1 and need of intravenous antibiotic therapy. CF patients in Moscow stratified by age groups had lower BMI than CF patients in Frankfurt (age 16-18: p=0,003; age 19-22: p=0,004; age 23-29: p<0,001; age 30-35: p<0,001; age 36-66: p=0,024). In a matching pairs analysis including 100 patients from Frankfurt and 100 patients from Moscow for the year 2015 FEV1 was significantly lower in Moscow patients (p<0,001).
Conclusion: BMI, FEV1 and need of intravenous therapy have significant impact on survival and on quality of life of CF patients. A lower BMI and a lower FEV1 result in a worse survival and determine the prognosis. This study showed a significant difference in prognostic parameters between Frankfurt and Moscow in the crosssectional analysis for the year 2015. A further study should evaluate this difference to show whether this difference will be found over a longer period of time.
Einleitung: Die akute Tonsillitis gehört zu den Infektionen der oberen Atemwege und ist eine sehr häufige Erkrankung in einer Kinder- und Jugendarztpraxis. Ziel unserer Untersuchung war es, das virale und bakterielle Erregerspektrum der akuten Tonsillitis, ihre saisonale Verteilung, ihre Altersverteilung, die klinische Symptomatik und den Einfluss einer Rauchexposition zu untersuchen. Gleichzeitig sollte erneut die Sensitivität und Spezifität des angewandten StrepA ST überprüft werden.
Methoden: In drei Kinder- und Jugendarztpraxen im Rhein-Main Gebiet wurden zwischen April 2009 und Mai 2010 insgesamt 1720 Patienten mit akuten Halsschmerzen untersucht. Mit einem Anamnesebogen wurden Alter, klinische Symptomatik und die Rauchexposition erfasst. Bei allen Patienten wurde ein StrepA ST durchgeführt. In einer Praxis (Praxis 1) wurden bei 306 Patienten zusätzlich ein Abstrich für eine bakterielle Kultur und für eine Multiplex PCR auf Viren durchgeführt.
Resultate: In 84% der Fälle tritt die GAS Tonsillitis im Alter zwischen 2 und 12 Jahren auf. Es konnte keine saisonale Häufung der GAS nachgewiesen werden. Mit 64 (42%) StrepA ST positiven Patienten von 152 Raucher-Familien und 74 (37%) von 200 Nichtraucher-Raucher-Familien zeigt sich kein signifikant erhöhtes Risiko an einer GAS Tonsillitis zu erkranken, wenn mindestens ein Elternteil raucht. Bei 306 Rachenabstrichen konnten 145 (47,5%) mal Streptokokken nachgewiesen werden. Davon waren mit 133 vorwiegend GAS (92%). Die anderen Streptokokken der Gruppen C (4,8%), G (2,1%) und B (1,4%) kommen deutlich seltener vor und spielen eine untergeordnete Rolle. Die Sensitivität und Spezifität des StrepA ST war mit 89,9% und 94,1% ausgezeichnet. Bei 306 Tonsillitiden gelang bei 110 Patienten (35,8%) ein Virusnachweis. Wie erwartet fanden sich doppelt so viele Virusnachweise (46%) bei Patienten ohne GAS Nachweis als Ko-Infektionen bei einer GAS (24%). Der Anteil der Entero-/Rhinoviren unter den nachgewiesenen Viren war mit 54% am höchsten. Adenoviren waren mit 15% und Influenza- 9% die nächst häufigen Viren, Para- und Coronaviren bildeten kleinere Gruppen. Während Entero-/Rhinoviren ganzjährig vorkommen sind Influenzaviren eher in der kalten Jahreszeit für akute Tonsillitiden verantwortlich. Die Rolle der Ko-Infektion in der Entstehung und im Verlauf der akuten Tonsillitis muss in weiteren Untersuchungen erforscht werden.
Access to specialized care is essential for people with Parkinson´s disease (PD). Given the growing number of people with PD and the lack of general practitioners and neurologists, particularly in rural areas in Germany, specialized PD staff (PDS), such as PD nurse specialists and Parkinson Assistants (PASS), will play an increasingly important role in the care of people with PD over the coming years. PDS have several tasks, such as having a role as an educator or adviser for other health professionals or an advocate for people with PD to represent and justify their needs. PD nurse specialists have been established for a long time in the Netherlands, England, the USA, and Scandinavia. In contrast, in Germany, distinct PDS models and projects have been established. However, these projects and models show substantial heterogeneity in terms of access requirements, education, theoretical and practical skills, principal workplace (inpatient vs. outpatient), and reimbursement. This review provides an overview of the existing forms and regional models for PDS in Germany. PDS reimbursement concepts must be established that will foster an implementation throughout Germany. Additionally, development of professional roles in nursing and more specialized care in Germany is needed.
Hintergrund: Patienten, die präoperativ an einer eisendefizitären Erythropoese (IDE) oder Anämie leiden, haben unabhängig von anderen Erkrankungen ein erhöhtes Risiko für postoperative Morbidität. Ein Eisenmangel ist der häufigste Grund für eine Anämie und kann, wenn er frühzeitig diagnostiziert wird, effizient mittels Eisensubstitution behandelt werden. Zink-Protoporphyrin (ZnPP) ist im Vergleich zu klassischen Parametern wie Ferritin ein vielversprechender Parameter, um eine IDE zu diagnostizieren. Bisher wurde der Parameter im Blut gemessen. Nun soll geprüft werden, ob eine nicht-invasive Messung valide Ergebnisse liefert.
Methoden: Von März 2017 bis April 2018 wurden am Universitätsklinikum Frankfurt Patienten, die für eine Operation mit einem erwarteten Blutverlust von >10% geplant waren, auf eine IDE untersucht. Die Messung von nicht-invasivem ZnPP (ZnPP-NI) wurde mit der ZnPP-Referenz-Messung des ZnPP/Häm-Verhältnisses mittels Hochleistungsflüssigchromatographie (ZnPP-HPLC) verglichen. Die analytische Performance beim Nachweis einer IDE wurde mit im Blut gemessenen klassischen Eisenstatusparameter (Ferritin, Transferrinsättigung [TSAT], löslicher Transferrinrezeptor [sTfR] und sTfR-Index [sTfR-F]) verglichen.
Ergebnis: In dieser prospektiven Studie konnten 285 chirurgische Patienten präoperativ untersucht werden. Die Limits of Agreement zwischen ZnPP-NI und ZnPP-HPLC betrugen 20,3 μmol/mol Häm (95% -Konfidenzintervall 18,0-21,3; Akzeptanzkriterien 24,4 μmol/mol Häm; absolute Bias -0,3 μmol/mol Häm). Die analytische Performance zum Nachweis einer IDE der im Blut gemessenen Parameter war: ZnPP-HPLC (0,95), sTfR (0,90), sTfR-F (0,89), ZnPP-NI (0,88), TSAT (0,87) und Ferritin (0,65).
Fazit: Beim Nachweis einer IDE ist ZnPP-NI besser geeignet als Ferritin und vergleichbar valide wie TSAT. Der Vergleich mit einem Multiparameter-Index-Test ergab, dass ZnPP-NI von ≤40 μmol/mol Häm den Ausschluss einer IDE ermöglicht und ein Wert von ≥65 μmol/mol Häm eine IDE wahrscheinlich macht. ZnPP-NI kann daher für eine schnelle Erstbewertung in der IDE-Diagnostik und im Anämie Management ohne Blutentnahme verwendet werden.
Aufgrund wachsender Evidenz zu guten Langzeitergebnissen und geringen Komplikationsraten gewinnt die Prostataarterienembolisation (PAE) in der Therapie des Benignen Prostatasyndroms an Bedeutung. Durch ihren hohen technischen Anspruch bedarf es im Vorfeld einer umfassenden Untersuchung der Beckengefäßanatomie. Das Vorliegen eines zum Teil jungen Patientenkollektivs rückt zudem das Einsparen von Strahlung in den Fokus. In diesem Rahmen gewinnt die Magnet-Resonanz-Angiografie (MRA) an Aufmerksamkeit. Obwohl bereits erste Studien Erfolg versprechen, wird die MRA zur PAE-Planung zum Teil kritisch betrachtet, da sie aufwändiger und in der Auflösung unterlegen sei. In dieser Arbeit wurde untersucht, welche Vorteile die MRA im Zuge der PAE-Planung bietet und ob die klinische Effektivität der PAE unbeeinträchtigt bleibt. Weiterhin wurde untersucht, ob eine erfolgreiche MRA-geführte Planung die benötigte Strahlendosis reduziert.
In diese retrospektive Analyse wurden 56 Patienten, die zwischen Januar 2017 und April 2018 im Frankfurter Institut für Diagnostische und Interventionelle Radiologie eine PAE erhielten und bei denen ein vollständiger, die Interventionszeit und Strahlungsparameter umfassender Datensatz sowie eine MRA vor der PAE vorlagen, eingeschlossen. Zusätzlich wurden mittels International Prostate Symptom Score (IPSS), Quality of Life (QoL) und International Index of Erectile Function (IIEF) klinische Daten vor und nach der PAE erhoben. In der Magnet-Resonanz-Tomografie (MRT) vor der PAE wurden das Prostatavolumen, die Intravesical Prostatic Protrusion (IPP) und der Prostatic Urethral Angle (PUA) untersucht. Zur Analyse der Prostataarterie wurden Maximum Intensity Projection (MIP) und ein dreidimensionales Modell verwendet. Um die Auswirkungen einer erfolgreichen Urspungsanalyse auf Interventionszeit und Strahlungsparameter zu untersuchen, wurden diese Faktoren zwischen zwei Gruppen verglichen. In der ersten Gruppe konnte die Prostataarterie mittels MRA ermittelt werden, in der zweiten Gruppe war dies v.a. aufgrund von technischen Mängeln der Bildakquisition nicht möglich.
Der Nachweis des Ursprungs gelang bei 84,73% (111 von 131) der Prostataarterien, davon entsprangen 52,25% der A. pudenda interna, 18,92% zusammen mit der A. vesicalis superior, 13,51% seltenen Ursprüngen, 10,81% der A. obturatoria und 4,51% der vorderen Division der A. iliaca interna unterhalb der A. vesicalis inferior. Die Gruppe mit erfolgreicher Ursprungsanalyse mittels MRA zeigte signifikant geringere Werte in Fluoroskopiezeit (-26,96%, p = 0,0282), Dosisflächenprodukt (-38,04%, p = 0,0025) und Eingangsdosis (-37,10%, p = 0,0020). Die PAE bedingte eine signifikante Verbesserung in IPSS (p < 0,0001), Lebensqualität (p < 0,0001) und IIEF (p = 0,0016), dabei konnte der von den Patienten angegebene IPSS-Wert um durchschnittlich 9,42 Punkte (-43,37%) und der QoL-Wert um 2 Punkte (-50,00%) reduziert werden. Das Prostatavolumen (p < 0,0001), IPP (p = 0,0004) und PUA (p < 0,0001) zeigten sich ebenfalls signifikant reduziert. Das Volumen der Prostata schrumpfte um 4,92 ml (-8,35%), die IPP um 1,2 mm (-9,2%) und der PUA um 5,5° (-8,10%). Signifikante Zusammenhänge konnten zwischen IPSS- und QoL-Reduktion (p < 0,0001, r = 0,7555), sowie zwischen Höhe des IPSS vor der PAE und der absoluten IPSS-Reduktion (p = 0,0041, r = -0,4434) nachgewiesen werden.
Die MRA ermöglicht eine strahlungsfreie Analyse des Abgangs der Prostataarterie. Durch diese Auswertung konnte die benötigte Strahlendosis signifikant reduziert werden. Die MRA-geplante PAE erzielte eine deutliche Verbesserung der Symptomatik und der Lebensqualität. Die erektile Funktion konnte signifikant verbessert werden. Prostatavolumen, IPP und PUA zeigten zwar signifikante Veränderungen, wiesen jedoch keinen Zusammenhang zu klinischen Entwicklungen auf. Zwischen dem Ausgangsvolumen der Prostata und dem klinischen Ergebnis konnte ebenfalls keine signifikante Korrelation festgestellt werden, jedoch scheint der Ausgangswert des IPSS eine prädiktive Funktion zu haben.
Die MRA-geplante PAE ist klinisch effektiv und ermöglicht durch die Analyse der Prostataarterie eine Reduktion der benötigten Strahlung. Zusammen mit der MRT unterstützt sie die Indikationsstellung und Planung der PAE.
Zielsetzung: Ziel dieser Studie war die Überprüfung der Machbarkeit einer softwaregestützten radiologischen Evaluation der Cageposition und Quantifizierung einer möglichen Cagemigration und -sinterung anhand computertomographisch gewonnener DICOM-Daten im Rahmen des Heilungsprozesses interkorporell fusionierter Patienten. Zusätzlich dazu wurde eine mögliche Korrelation zum Fusionsverhalten des Cages sowie zum klinischen Outcome der Patienten analysiert.
Material und Methoden: In den postoperativen CT Datensätzen von 67 Patienten nach monosegmentaler, dorsal instrumentierter TLIF wurde mithilfe der Software VGStudio Max die Cageposition bestimmt. Eine im postoperativen Verlauf eingetretene Lageänderung ≥ 1 mm bzw. ≥ 3° wurde hierbei als minimale Migration / Sinterung, eine Lageänderung ≥ 3 mm bzw. ≥ 10° als deutliche Migration / Sinterung des Cages gewertet. Um zu prüfen, ob das Migrations- und Sinterungsverhalten einen Einfluss auf die Osteogenese hat, erfolgte auf Basis der von Bridwell et al publizierten Fusionskriterien in den 12 Monate postoperativen CT-Aufnahmen eine Evaluation des Fusionstatus‘. Zur klinischen Beurteilung wurden der Oswestry Disability Index, die Visuelle Analogskala, der Schmerzmittelbedarf und der modifizierte Pationnaire Questionnaire der Patienten ausgewertet.
Ergebnisse: Die Messung der Cageposition mittels VGStudio Max ist eine präzise und reliable Methode zur Quantifizierung einer Cagemigration und -sinterung. Insgesamt war bei 85,1% der Patienten eine Migration (61,2% minimal, 23,9% deutlich) und bei 58,2% der Patienten eine Sinterung (32,8% minimal, 25,4% deutlich) des Cages nachweisbar. Radiologische Zeichen einer Pseudarthrose fanden sich bei 5 Patienten (7,5%). Die übrigen 92,5% der Patienten wiesen eine Grad I bzw. II Fusion auf.
Cagemigration und -sinterung hatten keinen signifikanten Einfluss auf das Fusionsverhalten und das klinische Outcome. Eine Korrelation zwischen Fusionsergebnis und klinischem Outcome bestand ebenfalls nicht.
Schlussfolgerung: Die Inzidenz der Cagemigration ist - unter Berücksichtigung auch geringfügiger Lageänderungen der Cages - deutlich höher als vorbeschrieben. Auf Basis des Migrations- bzw. Sinterungsverhaltens von Cages können jedoch keine Rückschlüsse auf das Fusionsergebnis gezogen werden. Als Kriterium in der Fusionsbeurteilung eignet sich der Nachweis einer Cagemigration bzw. -sinterung daher nicht in dem Ausmaß wie bisher vermutet.
Acute kidney injury (AKI) is still associated with high morbidity and mortality incidence rates, and also bears an elevated risk of subsequent chronic kidney disease. Although the kidney has a remarkable capacity for regeneration after injury and may recover completely depending on the type of renal lesions, the options for clinical intervention are restricted to fluid management and extracorporeal kidney support. The development of novel therapies to prevent AKI, to improve renal regeneration capacity after AKI, and to preserve renal function is urgently needed. The Special Issue covers research articles that investigated the molecular mechanisms of inflammation and injury during different renal pathologies, renal regeneration, diagnostics using new biomarkers, and the effects of different stimuli like medication or bacterial components on isolated renal cells or in vivo models. The Special Issue contains important reviews that consider the current knowledge of cell death and regeneration, inflammation, and the molecular mechanisms of kidney diseases. In addition, the potential of cell-based therapy approaches that use mesenchymal stromal/stem cells or their derivates is summarized. This edition is complemented by reviews that deal with the current data situation on other specific topics like diabetes and diabetic nephropathy or new therapeutic targets.
Ferroptosis, a newly discovered form of cell death mediated by reactive oxygen species (ROS) and lipid peroxidation, has recently been shown to have an impact on various cancer types; however, so far there are only few studies about its role in hepatocellular carcinoma (HCC). The delicate equilibrium of ROS in cancer cells has found to be crucial for cell survival, thus increased levels may trigger ferroptosis in HCC.In our study, we investigated the effect of different ROS modulators and ferroptosis inducers on a human HCC cell line and a human hepatoblastoma cell line. We identified a novel synergistic cell death induction by the combination of Auranofin and buthionine sulfoxime (BSO) or by Erastin and BSO at subtoxic concentrations. We found a caspase-independent, redox-regulated cell death, which could be rescued by different inhibitors of ferroptosis. Both cotreatments stimulated lipid peroxidation. All these findings indicated ferroptotic cell death. Both cotreatments affected the canonical ferroptosis pathway through GPX4 downregulation. We also found an accumulation of Nrf2 and HO-1, indicating an additional effect on the non-canonical pathway. Our results implicate that targeting these two main ferroptotic pathways simultaneously can overcome chemotherapy resistance in HCC.
Onkogene RAS-Mutationen zählen mit einem Vorkommen von ca. 25% zu häufigen Genmutationen in malignen Tumoren. Auch im Rhabdomyosarkom (RMS), dem häufigsten Weichteilsarkom im Kindesalter, findet sich eine hohe Rate an wiederkehrenden RAS-Signalwegmutationen. Dabei scheint ein Zusammenhang zwischen der RMS-Risikostratifizierung und dem Vorkommen von RAS-Mutationen zu bestehen. Da Hochrisiko-RMS im Vergleich zu anderen Tumorentitäten im Kindesalter immer noch mit einer unterdurchschnittlichen Prognose einhergehen, stellen RAS-Mutationen einen interessanten Angriffspunkt für eine zielgerichtete Tumortherapie dar. Hierzu soll diese Arbeit durch eine genauere Charakterisierung der Auswirkungen onkogener RAS-Gene auf das RMS beitragen. Verwendet wurden genetisch modifizierte RMS13 Zellen mit ektoper Expression der onkogenen RAS-Mutationen HRAS12V, KRAS12V oder NRAS12V. Eine bereits gut beschriebene Eigenschaft von RAS ist die Förderung der Zellproliferation. Daneben wurde auch beschrieben, dass RAS Einfluss auf den programmierten Zelltod nehmen und in Abhängigkeit vom zellulären Kontext pro- oder auch antiapoptotisch wirken kann. Daher stellte sich die Frage, welche Auswirkungen onkogene RAS-Mutationen in diesem Kontext auf Rhabdomy-osarkomzellen haben. In dieser Arbeit wird gezeigt, dass die ektope Expression von HRAS12V, KRAS12V oder NRAS12V in RMS13 Zellen zu einer gesteigerten Zellproliferation führt, im Hinblick auf die spontane Zelltodrate jedoch keine Veränderungen bewirkt. Damit stellt die erhöhte Proliferationsrate RAS-mutierter Rhabdomyosarkome einen wichtigen Unterschied zu entsprechenden Tumoren ohne solche Mutationen dar. Chemotherapeutika wie Etoposid und Doxorubicin, die besonders effektiv gegen hochproliferierende Zellen sind, zeigen jedoch keinen signifikanten Unterschied in ihrer Wirksamkeit gegen RMS13 Zellen in Anwesenheit von onkogenem RAS. Damit scheint ein selektives Eingreifen in die proliferationsfördernden Mechanismen nötig zu sein, um RAS-mutierte Zellen gezielt in ihrem Wachstum zu hemmen. Dies verdeutlicht die Notwendigkeit, spezifischer, gezielter Tumortherapien. Neben dem Einfluss auf das Zellwachstum wurden auch Veränderungen in der Redoxhomöostase untersucht. Bisherige indirekte Hinweise auf einen erhöhten oxidativen Stress im RMS in Anwesenheit von RAS-Mutationen können in dieser Arbeit durch den direkten Nachweis erhöhter ROS-Level in RAS-mutierten RMS13 Zellen bestätigt werden. Die akzelerierte ROS-Konzentration lässt vermuten, dass das Überleben von RMS-Zellen mit konstitutiver RAS-Aktivierung in besonderem Maße von antioxidativen Zellstrukturen abhängig sein könnte. Dies könnte sie sensibler gegenüber exogenen Stimuli machen, die zu einer weiteren Erhöhung des oxidativen Stresses führen. Als hervorzuhebendes Ergebnis zeigt diese Arbeit jedoch, dass die ektope Expression von HRAS12V, KRAS12V oder NRAS12V in RMS13 Zellen vor einem oxidativen Zelltod schützt. In Anwesenheit der RAS-Mutationen zeigen RMS13 Zellen einen signifikant geringeren Zellviabilitätsverlust gegenüber einem Eingriff in verschiedene Komponenten des antioxidativen Systems wie durch RSL3 (Glutathion-Peroxidase 4 Inhibitor), Erastin (indirekter Inhibitor der Glutathion-Synthese) oder Auranofin (Thioredoxin-Reduktase-Inhibitor). Dies steht im Gegensatz zu den Erstbeschreibungen, in denen für RSL3 und Erastin eine RAS-selektive Wirkung gezeigt wurde. Als Besonderheit kann der durch RSL3 oder Erastin hervorgerufene Zelltod der RMS13 Zellen als Ferroptose identifiziert werden. Hierbei handelt es sich um eine vor kurzem neu beschriebene Form von programmiertem, oxidativem und eisenabhängigem Zelltod. Diese Arbeit verdeutlicht somit, dass onkogene RAS-Mutationen im RMS gezielt in die Redoxregulation eingreifen, jedoch nur in bestimmten zellulären Kontexten für oxidative Stressoren zu sensibilisieren scheinen. Daneben weist diese Arbeit auch einen protektiven Effekt von onkogenem RAS gegenüber dem dualen PI3K/mTOR-Inhibitor PI-103 in RMS13 Zellen nach. Zusammengenommen deutet dies darauf hin, dass RAS selektiv Einfluss auf durch zytotoxische Stimuli hervorgerufenen Zelltod nimmt. Die Ergebnisse dieser Arbeit, insbesondere der Nachweis einer erhöhten Resistenz gegenüber oxidativen Stressoren in Anwesenheit onkogener RAS-Gene, leisten einen wichtigen Beitrag zur Entwicklung neuer zielgerichteter und selektiver RMS-Therapiestrategien.
Simple Summary: Pseudoprogression detection in glioblastoma patients remains a challenging task. Although pseudoprogression has only a moderate prevalence of 10–30% following first-line treatment of glioblastoma patients, it bears critical implications for affected patients. Non-invasive techniques, such as amino acid PET imaging using the tracer O-(2-[18F]-fluoroethyl)-L-tyrosine (FET), expose features that have been shown to provide useful information to distinguish tumor progression from pseudoprogression. The usefulness of FET-PET in IDH-wildtype glioblastoma exclusively, however, has not been investigated so far. Recently, machine learning (ML) algorithms have been shown to offer great potential particularly when multiparametric data is available. In this preliminary study, a Linear Discriminant Analysis-based ML algorithm was deployed in a cohort of newly diagnosed IDH-wildtype glioblastoma patients (n = 44) and demonstrated a significantly better diagnostic performance than conventional ROC analysis. This preliminary study is the first to assess the performance of ML in FET-PET for diagnosing pseudoprogression exclusively in IDH-wildtype glioblastoma and demonstrates its potential.
Abstract: Pseudoprogression (PSP) detection in glioblastoma remains challenging and has important clinical implications. We investigated the potential of machine learning (ML) in improving the performance of PET using O-(2-[18F]-fluoroethyl)-L-tyrosine (FET) for differentiation of tumor progression from PSP in IDH-wildtype glioblastoma. We retrospectively evaluated the PET data of patients with newly diagnosed IDH-wildtype glioblastoma following chemoradiation. Contrast-enhanced MRI suspected PSP/TP and all patients underwent subsequently an additional dynamic FET-PET scan. The modified Response Assessment in Neuro-Oncology (RANO) criteria served to diagnose PSP. We trained a Linear Discriminant Analysis (LDA)-based classifier using FET-PET derived features on a hold-out validation set. The results of the ML model were compared with a conventional FET-PET analysis using the receiver-operating-characteristic (ROC) curve. Of the 44 patients included in this preliminary study, 14 patients were diagnosed with PSP. The mean (TBRmean) and maximum tumor-to-brain ratios (TBRmax) were significantly higher in the TP group as compared to the PSP group (p = 0.014 and p = 0.033, respectively). The area under the ROC curve (AUC) for TBRmax and TBRmean was 0.68 and 0.74, respectively. Using the LDA-based algorithm, the AUC (0.93) was significantly higher than the AUC for TBRmax. This preliminary study shows that in IDH-wildtype glioblastoma, ML-based PSP detection leads to better diagnostic performance.
During the course of sepsis in critically ill patients, kidney dysfunction and damage are among the first events of a complex scenario toward multi-organ failure and patient death. Acute kidney injury triggers the release of lipocalin-2 (Lcn-2), which is involved in both renal injury and recovery. Taking into account that Lcn-2 binds and transports iron with high affinity, we aimed at clarifying if Lcn-2 fulfills different biological functions according to its iron-loading status and its cellular source during sepsis-induced kidney failure. We assessed Lcn-2 levels both in serum and in the supernatant of short-term cultured renal macrophages (MΦ) as well as renal tubular epithelial cells (TEC) isolated from either Sham-operated or cecal ligation and puncture (CLP)-treated septic mice. Total kidney iron content was analyzed by Perls’ staining, while Lcn-2-bound iron in the supernatants of short-term cultured cells was determined by atomic absorption spectroscopy. Lcn-2 protein in serum was rapidly up-regulated at 6 h after sepsis induction and subsequently increased up to 48 h. Lcn-2-levels in the supernatant of TEC peaked at 24 h and were low at 48 h with no change in its iron-loading. In contrast, in renal MΦ Lcn-2 was low at 24 h, but increased at 48 h, where it mainly appeared in its iron-bound form. Whereas TEC-secreted, iron-free Lcn-2 was associated with renal injury, increased MΦ-released iron-bound Lcn-2 was linked to renal recovery. Therefore, we hypothesized that both the cellular source of Lcn-2 as well as its iron-load crucially adds to its biological function during sepsis-induced renal injury.
Motivation: Calculating the magnitude of treatment effects or of differences between two groups is a common task in quantitative science. Standard effect size measures based on differences, such as the commonly used Cohen's, fail to capture the treatment-related effects on the data if the effects were not reflected by the central tendency. The present work aims at (i) developing a non-parametric alternative to Cohen’s d, which (ii) circumvents some of its numerical limitations and (iii) involves obvious changes in the data that do not affect the group means and are therefore not captured by Cohen’s d.
Results: We propose "Impact” as a novel non-parametric measure of effect size obtained as the sum of two separate components and includes (i) a difference-based effect size measure implemented as the change in the central tendency of the group-specific data normalized to pooled variability and (ii) a data distribution shape-based effect size measure implemented as the difference in probability density of the group-specific data. Results obtained on artificial and empirical data showed that “Impact”is superior to Cohen's d by its additional second component in detecting clearly visible effects not reflected in central tendencies. The proposed effect size measure is invariant to the scaling of the data, reflects changes in the central tendency in cases where differences in the shape of probability distributions between subgroups are negligible, but captures changes in probability distributions as effects and is numerically stable even if the variances of the data set or its subgroups disappear.
Conclusions: The proposed effect size measure shares the ability to observe such an effect with machine learning algorithms. Therefore, the proposed effect size measure is particularly well suited for data science and artificial intelligence-based knowledge discovery from big and heterogeneous data.
Hintergrund: Die Erstversorgung von Wunden und kleinere chirurgische Eingriffe gehören neben der hochspezialisierten Medizin zu den allgemein notwendigen Grundleistungen der Notfallversorgung in den Kliniken. Die Vergütung der ambulanten Notfallleistungen für gesetzlich Versicherte erfolgt derzeit nach dem Einheitlichen Bewertungsmaßstab (EBM), welchem die betriebswirtschaftliche Aufwandserfassung des niedergelassenen Sektors als Kalkulationsgrundlage dient. Krankenhäuser haben im Vergleich zu Arztpraxen wesentlich höhere Vorhaltungskosten.
Ziel der Arbeit: In dieser Arbeit wird das entstehende Kosten-Erlös-Verhältnis der ambulanten Wundversorgung in einer Notaufnahme durch die Vergütung nach EBM analysiert.
Material und Methoden: Die Daten wurden in der Notaufnahme des Universitätsklinikums Frankfurt am Main über 12 Monate erhoben. Eingeschlossen wurden alle Patienten, die in diesem Zeitraum eine Wundversorgung mittels Naht erhielten. Die Kosten wurden der Abrechnung nach EBM 01210 (bzw. 01212) mit der Zusatzpauschale für kleinchirurgische Eingriffe EBM 02301 gegenübergestellt.
Ergebnisse: Im Beobachtungszeitraum wurden 1548 Patienten versorgt; das entspricht 19,52 % aller unfallchirurgischen Fälle. Den Kosten einer Standardwundversorgung in Höhe von 45,40 € steht eine Vergütung von 31,83 € gegenüber. Die Berechnung des Gesamterlöses weist einen Defizitbetrag von 13,57 € pro ambulantem Fall auf; dies entspricht einem Jahresdefizit von 21.006,36 €.
Diskussion: Es konnte gezeigt werden, dass ohne Betrachtung der relevanten Vorhaltekosten in keinem Fall eine Kostendeckung erreicht werden kann.
Die bisherige Vergütung der ambulanten Wundversorgung nach EBM erscheint unzureichend. Eine Anpassung bzw. Zusatzvergütung scheint notwendig, um eine ausreichende Versorgungsqualität in Zukunft sicherstellen zu können.
Objective: Value frameworks in oncology have not been validated for the assessment of treatments in haematological malignancies, but to avoid overlaps and duplications it appears reasonable to build up experience on existing value frameworks, such as the European Society for Medical Oncology—Magnitude of Clinical Benefit Scale (ESMO-MCBS).
Methods: Here we present the results of the first feasibility testing of the ESMO-MCBS v1.1 for haematological malignancies based on the grading of 80 contemporary studies for acute leukaemia, chronic leukaemia, lymphoma, myeloma and myelodysplastic syndromes. The aims were (1) to evaluate the scorability of data, (2) to evaluate the reasonableness of the generated grades for clinical benefit using the current version and (3) to identify shortcomings in the ESMO-MCBS v1.1 that require amendments to improve the efficacy and validity of the scale in grading new treatments in the management of haematological malignancies.
Results: In general, the ESMO-MCBS v1.1 was found to be widely applicable to studies in haematological malignancies, generating scores that were judged as reasonable by European Hematology Association (EHA) experts. A small number of studies could either not be graded or were not appropriately graded. The reasons, related to the differences between haematological and solid tumour malignancies, are identified and described.
Conclusions: Based on the findings of this study, ESMO and EHA are committed to develop a version of the ESMO-MCBS that is validated for haematological malignancies. This development process will incorporate all of the usual stringencies for accountability of reasonableness that have characterised the development of the ESMO-MCBS including field testing, statistical modelling, evaluation for reasonableness and openness to appeal and revision. Applying such a scale will support future public policy decision-making regarding the value of new treatments for haematological malignancies and will provide insights that could be helpful in the design of future clinical trials.
MicroRNA miR-181 - a rheostat for TCR signaling in thymic selection and peripheral T-Cell function

The selection of T cells during intra-thymic d evelopment is crucial to obtain a functional and simultaneously not self-reactive peripheral T cell repertoire. However, selection is a complex process dependent on T cell receptor (TCR) thresholds that remain incompletely understood. In peripheral T cells, activation, clonal expansion, and contraction of the active T cell pool, as well as other processes depend on TCR signal strength. Members of the microRNA (miRNA) miR-181 family have been shown to be dynamically regulated during T cell development as well as dependent on the activation stage of T cells. Indeed, it has been shown that expression of miR-181a leads to the downregulation of multiple phosphatases, implicating miR-181a as ‘‘rheostat’’ of TCR signaling. Consistently, genetic models have revealed an essential role of miR-181a/b-1 for the generation of unconventional T cells as well as a function in tuning TCR sensitivity in peripheral T cells during aging. Here, we review these broad roles of miR-181 family members in T cell function via modulating TCR signal strength.
Background: Culture-derived mesenchymal stromal cells (MSCs) exhibit variable characteristics when manufactured using different methods and different source materials. The purpose of this study was to assess the impact on MSC characteristics when different laboratories propagated MSCs from cultures initiated with BM aliquots derived from the same donor source material.
Methods and Methods: Five aliquots from each of three different BM donors were distributed to five independent laboratories. Three laboratories plated whole BM and two laboratories a mononuclear BM cell fraction. Four laboratories cultured in media supplemented with fetal bovine serum (FBS) and one laboratory used human platelet lysate (hPL). Initial cell seeding densities (i.e., P0) ranged from 19.7 × 103/cm2–282 × 103/cm2 and for second seeding (i.e., P1) 0.05 × 103–5.1 × 103 cells/cm2. Post-thawed MSCs from each laboratory were analyzed for cell viability, immunophenotype, tri-lineage differentiation, fibroblast colony-forming units (CFU-F), gene expression, and immunosuppressive activity.
Results: Transit times from BM collection to receipt by laboratories located in the United States ranged from 16.0–30.0 h and from 41.5–71.5 h for a laboratory in Asia. Post-thaw culture derived MSCs rom BM #1, #2, and #3 exhibited viabilities that ranged from 74–92%, 61–96%, and 23–90%, respectively. CFU activity from BM #1, #2, and #3 per 200 MSCs plated averaged 45.1 ± 21.4, 49.3 ± 26.8 and 14.9 ± 13.3, respectively. No substantial differences were observed in immunophenotype, and immunosuppressive activities. Global gene expression profiles of MSCs revealed transcriptome differences due to different inter-laboratory methods and to donor source material with the center effects showing greater molecular differences than source material.
Conclusion: Functional and molecular differences exist among MSCs produced by different centers even when the same BM starting material is used to initiate cultures. These results indicated that manufacturing of MSCs by five independent centers contributed more to MSC variability than did the source material of the BM used in this study. Thus, emphasizing the importance of establishing worldwide standards to propagate MSCs for clinical use.
MicroRNAs (miRNAs) have emerged as critical posttranscriptional regulators of the immune system, including function and development of regulatory T (Treg) cells. Although this critical role has been firmly demonstrated through genetic models, key mechanisms of miRNA function in vivo remain elusive. Here, we review the role of miRNAs in Treg cell development and function. In particular, we focus on the question what the study of miRNAs in this context reveals about miRNA biology in general, including context-dependent function and the role of individual targets vs. complex co-targeting networks. In addition, we highlight potential technical pitfalls and state-of-the-art approaches to improve the mechanistic understanding of miRNA biology in a physiological context.
Proper speech production requires auditory speech feedback control. Models of speech production associate this function with the right cerebral hemisphere while the left hemisphere is proposed to host speech motor programs. However, previous studies have investigated only spectral perturbations of the auditory speech feedback. Since auditory perception is known to be lateralized, with right-lateralized analysis of spectral features and left-lateralized processing of temporal features, it is unclear whether the observed right-lateralization of auditory speech feedback processing reflects a preference for speech feedback control or for spectral processing in general. Here we use a behavioral speech adaptation experiment with dichotically presented altered auditory feedback and an analogous fMRI experiment with binaurally presented altered feedback to confirm a right hemisphere preference for spectral feedback control and to reveal a left hemisphere preference for temporal feedback control during speaking. These results indicate that auditory feedback control involves both hemispheres with differential contributions along the spectro-temporal axis.
Although immune checkpoint and targeted therapies offer remarkable benefits for lung cancer treatment, some patients do not qualify for these regimens or do not exhibit consistent benefit. Provided that lung cancer appears to be driven by transforming growth factor beta signaling, we investigated the single drug potency of Pirfenidone, an approved drug for the treatment of lung fibrosis. Five human lung cancer cell lines and one murine line were investigated for transforming growth factor beta inhibition via Pirfenidone by using flow cytometry, In-Cell western analysis, proliferation assays as well as comprehensive analyses of the transcriptome with subsequent bioinformatics analysis. Overall, Pirfenidone induced cell cycle arrest, down-regulated SMAD expression and reduced proliferation in lung cancer. Furthermore, cell stress pathways and pro-apoptotic signaling may be mediated by reduced expression of Survivin. A murine subcutaneous model was used to assess the in vivo drug efficacy of Pirfenidone and showed reduced tumor growth and increased infiltration of T cells and NK cells. This data warrant further clinical evaluation of Pirfenidone with advanced non-small cell lung cancer. The observed in vitro and in vivo effects point to a substantial benefit for using Pirfenidone to reactivate the local immune response and possible application in conjunction with current immunotherapies.
Biofabrication of SDF-1 functionalized 3D-printed cell-free scaffolds for bone tissue regeneration

Large segmental bone defects occurring after trauma, bone tumors, infections or revision surgeries are a challenge for surgeons. The aim of our study was to develop a new biomaterial utilizing simple and cheap 3D-printing techniques. A porous polylactide (PLA) cylinder was printed and functionalized with stromal-derived factor 1 (SDF-1) or bone morphogenetic protein 7 (BMP-7) immobilized in collagen type I. Biomechanical testing proved biomechanical stability and the scaffolds were implanted into a 6 mm critical size defect in rat femur. Bone growth was observed via x-ray and after 8 weeks, bone regeneration was analyzed with µCT and histological staining methods. Development of non-unions was detected in the control group with no implant. Implantation of PLA cylinder alone resulted in a slight but not significant osteoconductive effect, which was more pronounced in the group where the PLA cylinder was loaded with collagen type I. Addition of SDF-1 resulted in an osteoinductive effect, with stronger new bone formation. BMP-7 treatment showed the most distinct effect on bone regeneration. However, histological analyses revealed that newly formed bone in the BMP-7 group displayed a holey structure. Our results confirm the osteoinductive character of this 3D-biofabricated cell-free new biomaterial and raise new options for its application in bone tissue regeneration.
Ionizing radiation interacts with the immune system in many ways with a multiplicity that mirrors the complexity of the immune system itself: namely the need to maintain a delicate balance between different compartments, cells and soluble factors that work collectively to protect, maintain, and restore tissue function in the face of severe challenges including radiation damage. The cytotoxic effects of high dose radiation are less relevant after low dose exposure, where subtle quantitative and functional effects predominate that may go unnoticed until late after exposure or after a second challenge reveals or exacerbates the effects. For example, low doses may permanently alter immune fitness and therefore accelerate immune senescence and pave the way for a wide spectrum of possible pathophysiological events, including early-onset of age-related degenerative disorders and cancer. By contrast, the so called low dose radiation therapy displays beneficial, anti-inflammatory and pain relieving properties in chronic inflammatory and degenerative diseases. In this review, epidemiological, clinical and experimental data regarding the effects of low-dose radiation on the homeostasis and functional integrity of immune cells will be discussed, as will be the role of immune-mediated mechanisms in the systemic manifestation of localized exposures such as inflammatory reactions. The central conclusion is that ionizing radiation fundamentally and durably reshapes the immune system. Further, the importance of discovery of immunological pathways for modifying radiation resilience amongst other research directions in this field is implied.

According to UNAIDS, in 2018, about 35% of HIV-1 infected persons did not receive antiretroviral therapy (ART), resulting in 1.7 million new infections in that year...
Largely unnoticed, all life on earth is constantly exposed to low levels of ionizing radiation. Radon, an imperceptible natural occurring radioactive noble gas, contributes as the largest single fraction to radiation exposure from natural sources. For that reason, radon represents a major issue for radiation protection. Nevertheless, radon is also applied for the therapy of inflammatory and degenerative diseases in galleries and spas to many thousand patients a year. In either case, chronic environmental exposure or therapy, the effect of radon on the organism exposed is still under investigation at all levels of interaction. This includes the physical stage of diffusion and energy deposition by radioactive decay of radon and its progeny and the biological stage of initiating and propagating a physiologic response or inducing cancer after chronic exposure. The purpose of this manuscript is to comprehensively review the current knowledge of radon and its progeny on physical background, associated cancer risk and potential therapeutic effects.
Hintergrund
Obwohl Feedback ein wichtiges und gut untersuchtes Element der medizinischen Ausbildung darstellt, wird es trotz eines großen Bedarfs der Studierenden sowohl im Unterricht als auch in Prüfungssituationen nur selten angewendet. Die Frankfurter Medizinstudierenden beklagen besonders, dass sie zu ihren Objective Structured Clinical Examinations (OSCEs) als Abschlussprüfung im Fach Chirurgie bisher kein detailliertes Feedback erhalten. Auch die Prüfenden beklagen häufig, dass sie die Studierenden weder für herausragende Leistungen loben noch über auftretende Fehler informieren können.
Ziel dieser Arbeit ist deshalb die Erstellung und Implementierung eines strukturierten schriftlichen Feedbacks in eine bestehende OSCE-Prüfung im Fach Chirurgie, das an den Bedürfnissen sowohl der Studierenden als auch der Prüfenden orientiert ist.
Material und Methoden
Das Studiendesign war prospektiv. Im ersten Schritt wurde eine Befragung erfahrener OSCE-Prüfender durchgeführt, um zu erheben, welches Feedback sie gerne an Studierende weitergeben würden. Basierend hierauf wurde ein erster Feedbackbogen erstellt. Dieser umfasste neben vorformulierten Aussagen auch die Möglichkeit Freitextkommentare zu geben und wurde von den Prüfenden für jeden Studierenden in der Wechselzeit zwischen den OSCE-Stationen ausgefüllt. Die Feedbackbögen wurden anschließend eingescannt und per E- Mail an die Studierenden geschickt. Im Anschluss hieran erfolgte eine webbasierte Befragung der Studierenden und der OSCE-Prüfenden, sowie eine tiefergehende Befragung der Studierenden in Form von Fokusgruppen- Interviews.
Basierend auf den Ergebnissen der Umfragen und der Fokusgruppen wurden die Feedbackbögen nochmals grundlegend überarbeitet und im folgenden OSCE erneut angewendet. Die Zufriedenheit der Prüfenden und Studierenden wurde analog zur ersten Befragung erhoben.
Ergebnisse
Insgesamt nahmen 351 Studierende und 51 Prüfende in beiden OSCEs an der Studie teil. In der abschließenden Online-Evaluation gaben 87,5% der Studierenden und 91,6% der Prüfenden an, dass sie zustimmen oder eher zustimmen, dass das schriftliche Feedback in zukünftigen OSCE-Prüfungen weiterhin angewendet werden soll. Mehr als 50% der Studierenden gaben jedoch an, dass das Feedback noch nicht konkret genug sei.
Mehr als ein Viertel der Prüfenden gab an, dass das Ausfüllen der Feedbackbögen zeitlich herausfordernd sei. In allen Fokusgruppen wurde das schriftliche Feedback durch die Studierenden befürwortet.
Schlussfolgerung
Die Implementierung eines strukturierten schriftlichen Feedbacks in einen OSCE ist problemlos möglich. Das schriftliche Feedback wird sowohl von den Prüfenden als auch von den Studierenden als nützlich empfunden.
In der durchgeführten Studie erfolgte die Untersuchung des visuellen Arbeits-gedächtnisses von bipolaren Patienten im Vergleich zu gesunden Kontrollen. Es erfolgten bereits viele Untersuchungen an Patienten mit bipolarer Störung. Wird das Hauptaugenmerk auf die kognitiven Funktionen der Patienten gelegt, so konnte bereits in einigen Studien gezeigt werden, dass nicht nur in depressi-ver oder manischer, sondern auch in euthymer Stimmungslage kognitive Defizi-te vorliegen. Zur näheren Untersuchung der Funktionen des visuellen Arbeits-gedächtnisses der Patienten mit bipolarer Störung wurde daher eine fMRT-Untersuchung durchgeführt. Hier wurden Patienten, die an bipolarer Störung erkrankt sind, mit gesunden Kontrollen verglichen. Dabei wurden die bipolaren Patienten in euthymer Stimmungslage untersucht. Weder in Antwortrichtigkeit noch Reaktionsgeschwindigkeit konnte ein signifikanter Gruppenunterschied nachgewiesen werden. Außerdem wurde in der Untersuchung eine Differenzie-rung zwischen den einzelnen Phasen gemacht, die eine Gedächtnisinformation durchläuft. Bei diesen Phasen handelt es sich um Enkodierungs-, Halte- und Abrufphase. Hierbei konnten veränderte Aktivierungsmuster an diversen Hirn-strukturen der bipolaren Patienten dargestellt werden. Diese Veränderungen ziehen sich durch alle drei Phasen der Gedächtniskonsolidierung und können vor allem im präfrontalen Kortex nachgewiesen werden. Es handelt sich dabei vor allem um eine schwächere Aktivierung des präfrontalen Kortex (PFC) der bipolaren Patienten im Vergleich zu gesunden Kontrollen. Unter anderem ist das Arbeitsgedächtnis im PFC lokalisiert. Diese Ergebnisse scheinen ein Hin-weis dafür zu sein, dass bei den bipolaren Patienten neuronale Defizite im visu-ellen Arbeitsgedächtnis vorliegen.
Background: Data on Candida bloodstream infections in pediatric patients in Europe are limited. We performed a retrospective multicenter European study of the epidemiology and outcome of neonatal and pediatric candidemia.
Material and Methods: All first positive blood cultures from patients ≤ 18 years of age with candidemia were registered. Patients’ demographic and clinical characteristics and causative Candida species were collected and analyzed. Regression analysis was used to identify factors independently associated with mortality.
Results: One thousand three hundred ninety-five episodes of candidemia (57.8% male) were reported from 23 hospitals in 10 European countries. Of the 1395 episodes, 36.4% occurred in neonates (≤ 44 weeks postmenstrual age), 13.8% in infants (> 44 weeks postmenstrual age to 1 year) and 49.8% in children and adolescents. Candida albicans (52.5%) and Candida parapsilosis (28%) were the predominant species. A higher proportion of candidemia caused by C. albicans was observed among neonatal patients (60.2%) with highest rates of C. parapsilosis seen among infants (42%). Children admitted to hematology-oncology wards presented the highest rates of non-albicans Candida species. Candidemia because of C. albicans was more frequent than non-albicans Candida in Northern versus Southern Europe (odds ratio, 2.3; 95% confidence interval, 1.8–2.9; P < 0.001). The all-cause mortality at 30 days was 14.4%. All-cause mortality was higher among patients admitted to the neonatal or pediatric intensive care units than other wards. Over time, no significant changes in species distribution were observed.
Conclusions: This first multicenter European study shows unique characteristics of the epidemiology of pediatric candidemia. The insights obtained from this study will be useful to guide clinical management and antifungal stewardship.
Lyme disease (LD), which is caused by genospecies of the Borrelia burgdorferi sensu lato complex, is the most common vector-borne disease in the Northern hemisphere. Spirochetes are transmitted by Ixodes ticks and maintained in diverse vertebrate animal hosts. Following tick bite, spirochetes initially establish a localized infection in the skin. However, they may also disseminate hematogenously to several distal sites, including heart, joints, or the CNS. Because they need to survive in diverse microenvironments, from tick vector to mammalian hosts, spirochetes have developed multiple strategies to combat the numerous host defense mechanisms. One of these strategies includes the production of a number of complement-regulator acquiring surface proteins (CRASPs) which encompass CspA, CspZ, and OspE paralogs to blunt the complement pathway. These proteins are capable of preventing complement activation on the spirochete surface by binding to complement regulator Factor H. The genes encoding these CRASPs differ in their expression patterns during the tick-to-host infection cycle, implying that these proteins may exhibit different functions during infection. This review summarizes the recent published reports which investigated the roles that each of these molecules plays in conferring tick-borne transmission and dissemination in vertebrate hosts. These findings offer novel mechanistic insights into LD pathobiology and may facilitate the identification of new targets for preventive strategies against Lyme borreliosis.
Background: Aortic stenosis (AS) is the most prevalent primary heart valve disease and the third most prevalent cardiovascular disease in the world, having a severe and slowly progressive evolution. The implementation of Transcatheter Aortic Valve Implantation (TAVI) is necessary because the population is constantly ageing, and most elderly people have various comorbidities, which places them among patients with high surgical risk. The aim was to report immediate and 9 months follow-up results of first TAVI implantations in the Republic of Moldova.
Material and methods: Three patients with severe symptomatic AS and high surgical risk underwent TAVI implantation using Portico valves.
Results: The mean age was 76.7 ± 1.2 years, 2 women and 1 man. The mean Logistic EuroSCORE II was 5.68 ± 0.67%. Procedural success was achieved in all (100%) patients. After the intervention, it was shown that the GP mean decreased by an average of 20.27±1.61 mmHg, the Vmax of the jet through AoV decreased on average by 2.05±0.19 m/s, and they were maintained all over the nine months follow-up period. Quality of life, using the TASQ (Toronto Aortic Stenosis Quality of Life) questionnaire, was improved by 62.3 ± 2.2 points.
Conclusions: TAVI, as a new minimally invasive method of treatment for aortic valve stenosis appears to be safe, with a low rate of early and late complications in elderly patients at high surgical risk, with good clinical outcomes at 9 months. Its implementation in the Republic of Moldova is welcome in the conditions of continuously ageing population and the prevalence of associated comorbidities.
Background: Naturalistic developmental behavioural interventions (NDBI) have been shown to improve autism-specific symptoms in young children with Autism Spectrum Disorder (ASD). NDBI approaches, such as the ASD-specific Frankfurt Early Intervention Programme for ASD (A-FFIP), are based on ASD-specific developmental and learning aspects. A-FFIP is a low-intensity intervention which can easily be implemented in the local health care/social welfare system. The aim of the present study is to establish 1-year efficacy of the manualised early intervention programme A-FFIP in toddlers and preschool children with ASD. It is hypothesised that A-FFIP will result in improved ASD-specific symptoms compared to early intervention as usual (EIAU). Child- and family-specific secondary outcomes, as well as moderators and mediators of outcome, will be explored.
Methods/design: A prospective, multi-centre, parallel-group, randomised controlled, phase-III trial comparing A-FFIP versus EIAU. A total of 134 children (A-FFIP: 67, EIAU: 67) aged 24–66 months at baseline assessment meeting the criteria for ASD (DSM-5) will be included. The primary outcome is the absolute change of the total score of the Brief Observation of Social Communication Change (BOSCC-AT) between baseline (T2) and 1-year follow-up (T6). The treatment effect will be tested, adjusted for relevant covariates applying a mixed model for repeated measures. Secondary outcomes are BOSCC social communication and repetitive-behaviour scores, single ASD symptoms, language, cognition, psychopathology, parental well-being and family quality of life. Predictors, moderators and mediating mechanisms will be explored.
Discussion: If efficacy of the manualised A-FFIP early intervention is established, the current study has the potential to change clinical practice strongly towards the implementation of a low-intensity, evidence-based, natural early intervention in ASD. Early intervention in ASD requires specialist training, which subsequently needs to be developed or included into current training curricula.

Purpose: Sarcopenia, defined as a loss of muscle mass and quality, has been associated with impaired oncological outcome and treatment toxicities in several malignancies. However, its role in anal squamous cell carcinoma (ASCC) remains less well explored.
Methods/Materials: Planning CT scans were used to measure cross-sectional skeletal muscle area (SMA) to calculate the skeletal muscle index (SMI). The association of sarcopenia with clinical and treatment-related parameters, and toxicity was assessed in 114 patients with ASCC that underwent standard 5-Fluorouracil/Mitomycin C chemoradiotherapy (CRT). The prognostic impact of sarcopenia on local relapse-free survival (LRFS), disease-free survival (DFS), and overall survival was examined using a Cox regression analysis.
Results: 29 (25.4%) patients had sarcopenia. Patients with sarcopenia had lower baseline hemoglobin levels (p = 0.002), worse Karnofsky Performance Status (p = 0.001) lower BMI (p < 0.001), and a significantly lower body surface area (p = 0.03), and lower incidence of involved lymph nodes (p = 0.03). Regarding acute toxicity, sarcopenia was associated with a significantly higher incidence of ≥grade 3leukopenia (OR: 3.5; 95% CI: 1.6–7.5, p = 0.007) and ≥grade 3 thrombopenia (OR: 5.1; 95% CI: 1.3–21, p = 0.018) after CRT. Despite higher hematologic toxicity in sarcopenic patients, total treatment time was similar between patients with and without sarcopenia (median 44 vs 45 days, p = 0.95). There was no significant prognostic impact of sarcopenia on either LRFS, DFS, or OS.
Conclusion: This is the largest study to assess the impact of sarcopenia on toxicity and oncological outcome in patients with ASCC. Increased clinician awareness of higher hematological toxicity risk is needed for sarcopenic patients with ASCC undergoing CRT to facilitate closer monitoring of side effects and earlier introduction of supportive measures. Further prospective studies are needed to elucidate the prognostic role and impact of sarcopenia on CRT-related toxicity in ASCC.
Background: Radiotherapy dose and target volume prescriptions for anal squamous cell carcinoma (ASCC) vary considerably in daily practice and guidelines, including those from NCCN, UK, Australasian, and ESMO. We conducted a pattern-of-care survey to assess the patient management in German speaking countries.
Methods: We developed an anonymous questionnaire comprising 18 questions on diagnosis and treatment of ASCC. The survey was sent to 361 DEGRO-associated institutions, including 41 university hospitals, 118 non-university institutions, and 202 private practices.
Results: We received a total of 101 (28%) surveys, including 20 (19.8%) from university, 36 (35.6%) from non-university clinics, and 45 (44.6%) from private practices. A total of 28 (27.8%) institutions reported to treat more than 5 patients with early-stage ASCC and 42 (41.6%) institutions treat more than 5 patients with locoregionally-advanced ASCC per year. Biopsy of suspicious inguinal nodes was advocated in only 12 (11.8%) centers. Screening for human immunodeficiency virus (HIV) is done in 28 (27.7%). Intensity modulated radiotherapy or similar techniques are used in 97%. The elective lymph node dose ranged from 30.6 Gy to 52.8 Gy, whereas 87% prescribed 50.4–55. 8 Gy (range: 30.6 to 59.4 Gy) to the involved lymph nodes. The dose to gross disease of cT1 or cT2 ASCC ranged from 50 to ≥60 Gy. For cT3 or cT4 tumors the target dose ranged from 54 Gy to more than 60 Gy, with 76 (75.2%) institutions prescribing 59.4 Gy. The preferred concurrent chemotherapy regimen was 5-FU/Mitomycin C, whereas 6 (6%) prescribed Capecitabine/Mitomycin C. HIV-positive patients are treated with full-dose CRT in 87 (86.1%) institutions. First assessment for clinical response is reported to be performed at 4–6 weeks after completion of CRT in 2 (2%) institutions, at 6–8 weeks in 20 (19.8%), and 79 (78%) institutions wait up to 5 months.
Conclusions: We observed marked differences in radiotherapy doses and treatment technique in patients with ASCC, and also variable approaches for patients with HIV. These data underline the need for an consensus treatment guideline for ASCC.
Context: Autoimmune polyglandular syndrome (APS-2: autoimmune Addison’s disease or type 1 diabetes) is conferred by predisposing HLA molecules, vitamin D deficiency, and heritable susceptibility. Organ destruction is accompanied by cytokine alterations. We addressed the monocytic cytokines of two distinct APS-2 cohorts, effects of vitamin D and HLA DQ risk.
Methods: APS-2 patients (n = 30) and healthy controls (n = 30) were genotyped for HLA DQA1/DQB1 and their CD14+ monocytes stimulated with IL1β and/or 1,25(OH)2D3 for 24 h. Immune regulatory molecules (IL-6, IL-10, IL-23A, IL-15, CCL-2, PD-L1), vitamin D pathway gene transcripts (CYP24A1, CYP27B1, VDR), and CD14 were analyzed by enzyme-linked immunosorbent assay and RTqPCR. Results: Pro-inflammatory CCL-2 was higher in APS-2 patients than in controls (p = 0.001), whereas IL-6 showed a trend – (p = 0.1). In vitro treatment with 1,25(OH)2D3 reduced proinflammatory cytokines (IL-6, CCL-2, IL-23A, IL-15) whereas anti-inflammatory cytokines (IL-10 and PD-L1) rose both in APS-type 1 diabetes and APS-Addison´s disease. Patients with adrenal autoimmunity showed a stronger response to vitamin D. Expression of IL-23A and vitamin D pathway genes VDR and CYP27B1 varied by HLA genotype and was lower in healthy individuals with high-risk HLA (p = 0.0025; p = 0.04), while healthy controls with low-risk HLA showed a stronger IL-10 and CD14 expression (p = 0.01; p = 0.03). Conclusion: 1,25(OH)2D3 regulates the monocytic response in APS-2 disorders type 1 diabetes or Addison´s disease. The monocytic cytokine profile of individuals carrying HLA high-risk alleles is proinflammatory, enhances polyglandular autoimmunity and can be targeted by vitamin D.
Andropogon virginicus is an invasive weed that seriously threatens agricultural production and economics worldwide. In this research, dried aerial parts of A. virginicus were extracted, applying Soxhlet and liquid-liquid phase methods to acquire the total crude (T-Anvi), hexane (H-Anvi), ethyl acetate (E-Anvi), butanol (B-Anvi), and water (W-Anvi) extracts, respectively. In which, T-Anvi contains the highest total phenolic and flavonoid contents (24.80 mg gallic acid and 37.40 mg rutin equivalents per g dry weight, respectively). Via anti-radical (ABTS and DPPH), and reducing power assays, E-Anvi exhibits the most potent activities (IC50 = 13.96, 43.59 and 124.11 µg/mL, respectively), stronger than butylated hydroxytoluene (BHT), a standard antioxidant, while the lipid peroxidation inhibitory effect of E-Anvi (LPI = 90.85% at the concentration of 500 µg/mL) is close to BHT. E-Anvi shows the most substantial inhibition (IC50 = 2.58 mg/mL) on tyrosinase. Notably, α-amylase is significantly suppressed by H-Anvi (IC50 = 0.72 mg/mL), over twice stronger than the positive control, palmitic acid. In the cytotoxic assay, E-Anvi is the strongest extract inhibiting K562 cells (IC50 = 112.01 µg/mL). Meanwhile, T-Anvi shows the highest prevention on Meg-01 expansion (IC50 = 91.40 µg/mL). Dominant compounds detected in E-Anvi by high-performance liquid chromatography-electrospray ionization-tandem mass spectrometry (HPLC-ESI-MS/MS) are identified as flavonoids. However, among four major compounds identified in H-Anvi by gas chromatography-mass spectrometry (GC-MS), palmitic acid and phytol are the most abundant compounds with peak areas of 27.97% and 16.42%, respectively. In essence, this is the first report describing that A. virginicus is a potential natural source of antioxidants, tyrosinase and α-amylase inhibitors, and anti-chronic myeloid leukemia (CML) agents which may be useful in future therapeutics as promising alternative medicines.
Finding subgroups in biomedical data is a key task in biomedical research and precision medicine. Already one-dimensional data, such as many different readouts from cell experiments, preclinical or human laboratory experiments or clinical signs, often reveal a more complex distribution than a single mode. Gaussian mixtures play an important role in the multimodal distribution of one-dimensional data. However, although fitting of Gaussian mixture models (GMM) is often aimed at obtaining the separate modes composing the mixture, current technical implementations, often using the Expectation Maximization (EM) algorithm, are not optimized for this task. This occasionally results in poorly separated modes that are unsuitable for determining a distinguishable group structure in the data. Here, we introduce “Distribution Optimization” an evolutionary algorithm to GMM fitting that uses an adjustable error function that is based on chi-square statistics and the probability density. The algorithm can be directly targeted at the separation of the modes of the mixture by employing additional criterion for the degree by which single modes overlap. The obtained GMM fits were comparable with those obtained with classical EM based fits, except for data sets where the EM algorithm produced unsatisfactory results with overlapping Gaussian modes. There, the proposed algorithm successfully separated the modes, providing a basis for meaningful group separation while fitting the data satisfactorily. Through its optimization toward mode separation, the evolutionary algorithm proofed particularly suitable basis for group separation in multimodally distributed data, outperforming alternative EM based methods.
Communication with the hematopoietic system is a vital component of regulating brain function in health and disease. Traditionally, the major routes considered for this neuroimmune communication are by individual molecules such as cytokines carried by blood, by neural transmission, or, in more severe pathologies, by the entry of peripheral immune cells into the brain. In addition, functional mRNA from peripheral blood can be directly transferred to neurons via extracellular vesicles (EVs), but the parameters that determine their uptake are unknown. Using varied animal models that stimulate neuronal activity by peripheral inflammation, optogenetics, and selective proteasome inhibition of dopaminergic (DA) neurons, we show that the transfer of EVs from blood is triggered by neuronal activity in vivo. Importantly, this transfer occurs not only in pathological stimulation but also by neuronal activation caused by the physiological stimulus of novel object placement. This discovery suggests a continuous role of EVs under pathological conditions as well as during routine cognitive tasks in the healthy brain.
