Purpose: Acute kidney injury (AKI) is a severe complication in medical and surgical intensive care units accounting for a high morbidity and mortality. Incidence, risk factors, and prognostic impact of this deleterious condition are well established in this setting. Data concerning the neurocritically ill patients is scarce. Therefore, aim of this study was to determine the incidence of AKI and elucidate risk factors in this special population.
Methods: Patients admitted to a specialized neurocritical care unit between 2005 and 2011 with a length of stay above 48 hours were analyzed retrospectively for incidence, cause, and outcome of AKI (AKI Network-stage ≥2).
Results: The study population comprised 681 neurocritically ill patients from a mixed neurosurgical and neurological intensive care unit. The prevalence of chronic kidney disease (CKD) was 8.4% (57/681). Overall incidence of AKI was 11.6% with 36 (45.6%) patients developing dialysis-requiring AKI. Sepsis was the main cause of AKI in nearly 50% of patients. Acute kidney injury and renal replacement therapy are independent predictors of worse outcome (hazard ratio [HR]: 3.704; 95% confidence interval [CI]: 1.867-7.350; P < .001; and HR: 2.848; CI: 1.301-6.325; P = .009). Chronic kidney disease was the strongest independent risk factor (odds ratio: 12.473; CI: 5.944-26.172; P < .001), whereas surgical intervention or contrast agents were not associated with AKI.
Conclusions: Acute kidney injury in neurocritical care has a high incidence and is a crucial risk factor for mortality independently of the underlying neurocritical condition. Sepsis is the main cause of AKI in this setting. Therefore, careful prevention of infectious complications and considering CKD in treatment decisions may lower the incidence of AKI and hereby improve outcome in neurocritical care.
Although there are many dialect speakers in Bavaria, the dialect - mainly because of its non-standardized spelling - is usually not used in common print media or on nationwide television. Nevertheless, the Bavarian dialect appears on Bavarian television (BR) and in cinema films. However, the Bavarian used on television or in films is frequently not a genuine dialect; instead it is a synthetic language which resembles the German standard and merely refers to the dialect. This is mainly due to the needs of non-dialect speakers, who would definitely have comprehension problems with the genuine dialect. Furthermore, the Bavarian dialect is often used on online platforms, such as Facebook or YouTube. In these conversational situations, face-to-face communication is replaced by written texts. In the case of dialect speakers, these texts can appear as written dialect; due to the non-standardized spelling, the texts are strongly individualized.
The aim of this study is to trace back the translator of "Reinhold Lubenau Seyahatnamesi [Osmanlı Ülkesinde, 1587-1589]" and focus on her translation approach through paratexts. The traveller portrays the Muslim 'other' whom he met in the Ottoman Empire where he spent his time between the years 1587-1589, when the Ottoman Empire had the power and Islam was being perceived as a threat to them versus his own culture which he belonged to as a Prussian Protestant. The translation of this itinerary is available to Turkish readers after approximately 400 years in 2012. What makes this translation interesting is the translation of a source text, in which the target culture was being portrayed from the perspective of the 'other', into a target language. How was this 'foreign' perspective constructed by the traveler translated and reflected in the paratext? What was the approach of the translator against the challenges he encountered during the translation process? Answers to these questions among many others were being searched through an examination of paratexts. In addition, it was also discussed whether the author moved to the reader or the reader moved to the author. Paratexts encourage reading and direct the reception (Genette 2016). In this study, book covers, names, titles, genre, graphics illustration and footnotes were examined. Translator footnotes, which were provided by the translator in order to make the text clear, were classified in order to underline the functions of these footnotes. Translator footnotes are the tools, which make translators visible and help them to raise their voices in the texts. In peritexts, translator can provide extra information to the readers, explain and justify his/her translation decisions.
Aus einem ultranationalistischen Blickwinkel skizzierte Kuffner auf 32 Seiten und fünf beigefügten Karten die aus seiner Sicht notwendige territoriale Neuordnung Europas, die sowohl die allslawische Frage "od Šumavy až po Bajkal či Tichý Oceán" [vom Böhmerwald bis zum Baikalsee bzw. Pazifik] lösen als auch einen dauerhaften Frieden garantieren sollte. Die Hauptrolle dabei hatte er einem neu zu formierenden Staat zugedacht, dem er zwar den Namen "Čechy" (Böhmen) gab, der jedoch nicht nur die historischen Grenzen des Königreichs, sondern auch diejenigen der 1918 gegründeten ČSR deutlich überschritt. [...] Mit dieser überzogenen Denkschrift sollte Kuffer (unbeabsichtigt) einer antitschechischen Propaganda dies- und jenseits der 1919 sanktionierten Grenzen für 20 Jahre Dauermunition liefern: Binnen weniger Jahre avancierte sie zum meistzitierten tschechischen Werk im völkisch-deutschnationalen Schrifttum der 1920er und 1930er Jahre. Man glaubte in Kuffners Schrift "das heimliche Grundkonzept des tschechischen Imperialismus" aufgedeckt zu haben.
Konflikte erfüllen eine wesentliche Funktion in der Zusammenarbeit der Menschen. Sie werden meistens allgemein nur als negativ angesehen, sie können jedoch auch positive Funktionen haben. Dieser Beitrag fokussiert sich auf die Rolle des Translators als "Mittler/Mediator" und der Translation als eines Produkts, d. h. eines zielsprachlichen Textes, der auf der Grundlage eines ausgangssprachlichen Textes entstanden ist, in Bezug auf verschiedene mögliche Konfliktsituationen. Im Zentrum der Aufmerksamkeit steht auch die interkulturelle Funktion der Übersetzung als einer sog. sprachlichen "Mittlerin" zwischen unterschiedlichen Welten und Kulturen in der heutigen globalisierten Welt.
"Eames-the architect and the painter" ist ein Dokumentarfilm über das Paar Ray und Charles Eames, der sowohl ihr Arbeits-, als auch ihr Privatleben vorstellt. Der Film erzählt über die sehr eindrucksvolle Schaffenskraft und Arbeitsweise des kreativen Paares, das in seiner Werkstatt, dem Design Office Eames, ab Mitte der 1940er Jahre vor allem eine neue, moderne Formensprachen für Möbel suchte. Ihr Hauptanliegen galt von Beginn an der seriellen Anfertigung von Stühlen und Möbeln, die den neuen Ansprüchen des nach Aufbruch strebenden Zeitgeistes gerecht werden sollten. Dementsprechend wichtig war es ihnen, einen Ort zu etablieren, in dem, fern jeglicher Dogmatik und gängiger Arbeitsweisen, experimentell und freigeistig gearbeitet werden konnte. ...

Are stereotypes possible pillars supporting the learning of a foreign language or are they possible sources of intercultural misunderstandings? In most coursebooks, intercultural materials are embedded in teaching units. The understanding of others is closely related to the understanding of someone's own culture as well as the target culture. Only when someone is aware of the own culture, then the target culture can be understood. In foreign language learning, it plays an important role, so that the knowledge of language structures alone is not enough to use the language successfully. At the same time, it requires cultural knowledge of the target language culture. This study focuses on the effects of culture-based fossilized prejudices on foreign language learning and to show the quantity of these stereotypes in given coursebooks. A coursebook analysis reveals to what extent stereotypes can be found in the respective GFL and TFL coursebooks. A compile of given stereotypes create an overall picture of foreign cultures. To what extent this overall picture reflects the reality has been handled at the end of this study
