En tentant de regarder une image singulière avec quelques mots de Walter Benjamin, je voudrais, non pas me livrer à un exercice de commentaire, de glose ou de déchiffrement, mais, plus modestement, faire acte de reconnaissance, porter témoignage d’une experience de l’image, d’une 'poétique de l’image'. Walter Benjamin, l’un des penseurs qui a le plus radicalement bouleversé - réinventé - la notion de lisibilité (Lesbarkeit), est aussi l’un des penseurs au contact duquel l’histoire de l’art se trouve bouleversée dans son rapport même à la visibilité (Sichtbarkeit) et à la temporalité des images. Il ne s’agit pas seulement de rendre hommage à celui qui, mieux que quiconque, aura théorisé le rôle de la reproductibilité, taconté l’histoire de "l’art en tant que photographie" ou décrit le "déclin de l’aura". Il s’agit de travailler avec sa façon de mettre le temps au coeur de toute question d’image, et de faire de l’image - à commencer par la fameuse "image dialectique" - l’opérateur central de toute historicité. Le meilleur hommage, la meilleure justice que l’on puisse rendre à quelque chose, dit Benjamin - en parlant des "guenilles", des "rebuts" de l’histoire -, c’est de l’utiliser. Je vais tâcher de suivre cette leçon en vous présentant un objet de travail et en essayant de restituer la 'valeur d’usage' d’un ou de deux textes de Benjamin pour tenter de savoir, tout simplement, 'comment regarder une image'.

Die Verbindlichkeit von Patientenverfügungen ist umstritten. Derzeit wird in der öffentlichen und in der politischen Diskussion heftig hierüber debattiert. Der Deutsche Bundestag beabsichtigt, Gesetzesentwürfe zum Thema erneut zu diskutieren. Die Frage, wie die Wünsche von Patienten am Lebensende, wenn sie selbst nicht mehr entscheiden können, bei der Entscheidung über das Maß medizinischer Behandlungen am Lebensende berücksichtigt werden können, beschäftigt die Medizinethik und die Rechtswissenschaft seit vielen Jahren. Dem heutigen Stand der Diskussionen über Patientenverfügungen liegt die Mutmaßung zugrunde, dass eine Mehrzahl von Personen Patientenverfügungen als geeignetes Instrument erachten, die Autonomie am Lebensende im Falle der Unfähigkeit zur eigenen Entscheidung zu sichern. Aus diesem Grund wird die allgemeine Verbindlichkeit von Patientenverfügungen gefordert, um die Rechte der Patienten auch für das Lebensende zu sichern. Allerdings fehlten bislang empirische Untersuchungen, die diese Annahme belegen. Die Diskussion über Patientenverfügungen wird kontrovers geführt. Vertreter verschiedener rechtlicher, ethischer und auch medizinischer Positionen äußern Forderungen und Ansichten, die sich überschneiden oder aber gegensinnlich sind. Dies ist leicht verständlich, denn das Ausmaß der medizinischen Handlung am Lebensende versteht sich nicht von selbst. Die vorliegende Untersuchung beabsichtigte, diese Frage sozialempirisch zu beantworten. Ziel der Studie war es, empirische Daten und Informationen unter anderem zu folgenden Fragestellungen zu erheben: • Was sind die Wünsche und Präferenzen im Blick auf die inhaltliche Ausgestaltung von Patientenverfügungen, vornehmlich zur Einstellung bezüglich spezifischer Behandlungsoptionen am Lebensende? • Welche Wünsche und Präferenzen in Bezug auf den Personenkreis, der in Fragen der Gesundheitsfürsorge als Stellvertreter bevorzugt wird, haben die Befragten? • Wie hoch ist der Verbreitungsgrad von Patientenverfügungen, die Absicht eine Patientenverfügung zu verfassen und die Kenntnisse über die verschiedenen in Deutschland gültigen Formen von Vorsorgeverfügungen? • Bestehen Barrieren und Hindernisse, welche dem Verfassen einer Patientenverfügung entgegenstehen können? • Wie ist die Einschätzung der Verbindlichkeit von Patientenverfügungen anderer Personen, das heißt die Bereitschaft bzw. die ethische Verpflichtung, den schriftlich oder mündlich hinterlegten Verfügungen anderer Personen im Hinblick auf Behandlungen am Lebensende Folge zu leisten (der ethischen Verpflichtungen, nicht einer angenommen gesetzlichen Obliegenheit)? • Gibt es Determinanten, die die Verbreitung und Akzeptanz des Instrumentes Patientenverfügung beeinflussen können (demografische Daten, Selbsteinschätzungen des Gesundheitszustands und andere mehr)? In der hier vorgelegten Studie wurden 1260 Patientinnen und Patienten im Alter von 16 bis 99 Jahren, unter Zuhilfenahme eines standardisierten Fragebogens interviewt. Die Probanden wurden rekrutiert aus den Patienten, die hausärztliche Praxen im Rhein-Main Gebiet aufsuchten. Die Rekrutierung erstreckte sich über den Zeitraum vom 11.04.2006 bis zum 06.10.2006. Ergebnisse: 1. Ein relevanter Anteil von Befragten erweist sich als unsicher, Entscheidungen über lebenserhaltende Therapien im Voraus zu treffen. Diese Unsicherheit selbst könnte ein Grund sein, eine Patientenverfügung nicht auszufüllen. 2. Die Mehrheit der Befragten bevorzugt als Stellvertreter im Falle eigener Entscheidungsunfähigkeit das Zusammenwirken von Ärzten und Angehörigen. 3. Kenntnisse über Patientenverfügungen sind weit verbreitet. Dennoch füllt nur eine Minderheit solche Dokumente aus. Kenntnisse über die Vorsorgevollmacht oder die Betreuungsverfügung hingegen sind weniger verbreitet. 4. Wichtige Barrieren die dem Ausfüllen einer Patientenverfügung entgegenstehen sind: die Unsicherheit solche Entscheidungen im Voraus zu treffen (siehe oben), weiterhin verschiedenste Ängste vor dem Missbrauch dieser Dokumente. 5. Die Befragten erachten in einer großen Mehrheit Patientenverfügungen anderer nicht als verbindlich. Konfrontiert mit einem ethischen Dilemma wird in der Mehrzahl der Fälle für den Lebenserhalt plädiert. Auch die Einstellung der Befragten zu verschiedenen Behandlungsoptionen für das Lebensende generell und die schlussendlich in einem Fallbeispiel gewählte Handlung differieren deutlich. 6. Lediglich das Lebensalter der Befragten (ältere Menschen hatten eher eine Patientenverfügung) und das Vorhandensein von Kindern hat einen signifikanten Einfluss auf das Vorhandensein von Patientenverfügungen. Die in der vorliegenden Untersuchung erhobenen Ergebnisse können in der aktuellen Diskussion über Patientenverfügungen und deren Verbindlichkeit fruchtbar sein. Sie widersprechen weitverbreiteten Annahmen. Daher haben diese Ergebnisse auch in wissenschaftlichen Symposien und in der politischen Diskussion schon weite Beachtung gefunden (u.a. Posterpreise der Deutschen Gesellschaft für Innere Medizin und der Deutschen Krebsgesellschaft anlässlich ihrer Jahreskongresse).
Bayesian learning provides the core concept of processing noisy information. In standard Bayesian frameworks, assessing the price impact of information requires perfect knowledge of news’ precision. In practice, however, precision is rarely dis- closed. Therefore, we extend standard Bayesian learning, suggesting traders infer news’ precision from magnitudes of surprises and from external sources. We show that interactions of the different precision signals may result in highly nonlinear price responses. Empirical tests based on intra-day T-bond futures price reactions to employment releases confirm the model’s predictions and show that the effects are statistically and economically significant.
Background Production of interferon (IFN)-gamma is key to efficient anti-tumor immunity. The present study was set out to investigate effects of IFNgamma on the release of the potent pro-angiogenic mediator IL-8 by human A549 lung carcinoma cells.
Methods A549 cells were cultured and stimulated with interleukin (IL)-1beta alone or in combination with IFNgamma. IL-8 production by these cells was analyzed with enzyme linked immuno sorbent assay (ELISA). mRNA-expression was analyzed by real-time PCR and RNase protection assay (RPA), respectively. Expression of inhibitor-kappaBalpha, cellular IL-8, and cyclooxygenase-2 was analyzed by Western blot analysis.
Results Here we demonstrate that IFNgamma efficiently reduced IL-8 secretion under the influence of IL-1beta. Surprisingly, real-time PCR analysis and RPA revealed that the inhibitory effect of IFNgamma on IL-8 was not associated with significant changes in mRNA levels. These observations concurred with lack of a modulatory activity of IFNgamma on IL-1beta-induced NF-kappaB activation as assessed by cellular IkappaB levels. Moreover, analysis of intracellular IL-8 suggests that IFNgamma modulated IL-8 secretion by action on the posttranslational level. In contrast to IL-8, IL-1beta-induced cyclooxygenase-2 expression and release of IL-6 were not affected by IFNgamma indicating that modulation of IL-1beta action by this cytokine displays specificity.
Conclusions Data presented herein agree with an angiostatic role of IFNgamma as seen in rodent models of solid tumors and suggest that increasing T helper type 1 (Th1)-like functions in lung cancer patients e.g. by local delivery of IFNgamma may mediate therapeutic benefit via mechanisms that potentially include modulation of pro-angiogenic IL-8.
Histone deacetylase (HDAC) inhibitors represent a promising class of antineoplastic agents which affect tumour growth, differentiation and invasion. The effects of the HDAC inhibitor valproic acid (VPA) were tested in vitro and in vivo on pre-clinical renal cell carcinoma (RCC) models. Caki-1, KTC-26 or A498 cells were treated with various concentrations of VPA during in vitro cell proliferation 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assays and to evaluate cell cycle manipulation. In vivo tumour growth was conducted in subcutaneous xenograft mouse models. The anti-tumoural potential of VPA combined with low-dosed interferon-α (IFN-α) was also investigated. VPA significantly and dose-dependently up-regulated histones H3 and H4 acetylation and caused growth arrest in RCC cells. VPA altered cell cycle regulating proteins, in particular CDK2, cyclin B, cyclin D3, p21 and Rb. In vivo, VPA significantly inhibited the growth of Caki-1 in subcutaneous xenografts, accompanied by a strong accumulation of p21 and bax in tissue specimens of VPA-treated animals. VPA–IFN-α combination markedly enhanced the effects of VPA monotherapy on RCC proliferation in vitro, but did not further enhance the anti-tumoural potential of VPA in vivo. VPA was found to have profound effects on RCC cell growth, lending support to the initiation of clinical testing of VPA for treating advanced RCC.
The purpose of this phase III clinical trial was to compare two different extracellular contrast agents, 1.0 M gadobutrol and 0.5 M gadopentate dimeglumine, for magnetic resonance imaging (MRI) in patients with known or suspected focal renal lesions. Using a multicenter, single-blind, interindividual, randomized study design, both contrast agents were compared in a total of 471 patients regarding their diagnostic accuracy, sensitivity, and specificity to correctly classify focal lesions of the kidney. To test for noninferiority the diagnostic accuracy rates for both contrast agents were compared with CT results based on a blinded reading. The average diagnostic accuracy across the three blinded readers (‘average reader’) was 83.7% for gadobutrol and 87.3% for gadopentate dimeglumine. The increase in accuracy from precontrast to combined precontrast and postcontrast MRI was 8.0% for gadobutrol and 6.9% for gadopentate dimeglumine. Sensitivity of the average reader was 85.2% for gadobutrol and 88.7% for gadopentate dimeglumine. Specificity of the average reader was 82.1% for gadobutrol and 86.1% for gadopentate dimeglumine. In conclusion, this study documents evidence for the noninferiority of a single i.v. bolus injection of 1.0 M gadobutrol compared with 0.5 M gadopentate dimeglumine in the diagnostic assessment of renal lesions with CE-MRI.
Driving can be dangerous. Humans become inattentive when performing a monotonous task like driving. Also the risk implied while multi-tasking, like using the cellular phone while driving, can break the concentration of the driver and increase the risk of accidents. Others factors like exhaustion, nervousness and excitement affect the performance of the driver and the response time. Consequently, car manufacturers have developed systems in the last decades which assist the driver under various circumstances. These systems are called driver assistance systems. Driver assistance systems are meant to support the task of driving, and the field of action varies from alerting the driver, with acoustical or optical warnings, to taking control of the car, such as keeping the vehicle in the traffic lane until the driver resumes control. For such a purpose, the vehicle is equipped with on-board sensors which allow the perception of the environment and/or the state of the vehicle. Cameras are sensors which extract useful information about the visual appearance of the environment. Additionally, a binocular system allows the extraction of 3D information. One of the main requirements for most camera-based driver assistance systems is the accurate knowledge of the motion of the vehicle. Some sources of information, like velocimeters and GPS, are of common use in vehicles today. Nevertheless, the resolution and accuracy usually achieved with these systems are not enough for many real-time applications. The computation of ego-motion from sequences of stereo images for the implementation of driving intelligent systems, like autonomous navigation or collision avoidance, constitutes the core of this thesis. This dissertation proposes a framework for the simultaneous computation of the 6 degrees of freedom of ego-motion (rotation and translation in 3D Euclidean space), the estimation of the scene structure and the detection and estimation of independently moving objects. The input is exclusively provided by a binocular system and the framework does not call for any data acquisition strategy, i.e. the stereo images are just processed as they are provided. Stereo allows one to establish correspondences between left and right images, estimating 3D points of the environment via triangulation. Likewise, feature tracking establishes correspondences between the images acquired at different time instances. When both are used together for a large number of points, the result is a set of clouds of 3D points with point-to-point correspondences between clouds. The apparent motion of the 3D points between consecutive frames is caused by a variety of reasons. The most dominant motion for most of the points in the clouds is caused by the ego-motion of the vehicle; as the vehicle moves and images are acquired, the relative position of the world points with respect to the vehicle changes. Motion is also caused by objects moving in the environment. They move independently of the vehicle motion, so the observed motion for these points is the sum of the ego-vehicle motion and the independent motion of the object. A third reason, and of paramount importance in vision applications, is caused by correspondence problems, i.e. the incorrect spatial or temporal assignment of the point-to-point correspondence. Furthermore, all the points in the clouds are actually noisy measurements of the real unknown 3D points of the environment. Solving ego-motion and scene structure from the clouds of points requires some previous analysis of the noise involved in the imaging process, and how it propagates as the data is processed. Therefore, this dissertation analyzes the noise properties of the 3D points obtained through stereo triangulation. This leads to the detection of a bias in the estimation of 3D position, which is corrected with a reformulation of the projection equation. Ego-motion is obtained by finding the rotation and translation between the two clouds of points. This problem is known as absolute orientation, and many solutions based on least squares have been proposed in the literature. This thesis reviews the available closed form solutions to the problem. The proposed framework is divided in three main blocks: 1) stereo and feature tracking computation, 2) ego-motion estimation and 3) estimation of 3D point position and 3D velocity. The first block solves the correspondence problem providing the clouds of points as output. No special implementation of this block is required in this thesis. The ego-motion block computes the motion of the cameras by finding the absolute orientation between the clouds of static points in the environment. Since the cloud of points might contain independently moving objects and outliers generated by false correspondences, the direct computation of the least squares might lead to an erroneous solution. The first contribution of this thesis is an effective rejection rule that detects outliers based on the distance between predicted and measured quantities, and reduces the effects of noisy measurement by assigning appropriate weights to the data. This method is called Smoothness Motion Constraint (SMC). The ego-motion of the camera between two frames is obtained finding the absolute orientation between consecutive clouds of weighted 3D points. The complete ego-motion since initialization is achieved concatenating the individual motion estimates. This leads to a super-linear propagation of the error, since noise is integrated. A second contribution of this dissertation is a predictor/corrector iterative method, which integrates the clouds of 3D points of multiple time instances for the computation of ego-motion. The presented method considerably reduces the accumulation of errors in the estimated ego-position of the camera. Another contribution of this dissertation is a method which recursively estimates the 3D world position of a point and its velocity; by fusing stereo, feature tracking and the estimated ego-motion in a Kalman Filter system. An improved estimation of point position is obtained this way, which is used in the subsequent system cycle resulting in an improved computation of ego-motion. The general contribution of this dissertation is a single framework for the real time computation of scene structure, independently moving objects and ego-motion for automotive applications.
In the context of information theory, the term Mutual Information has first been formulated by Claude Elwood Shannon. Information theory is the consistent mathematical description of technical communication systems. To this day, it is the basis of numerous applications in modern communications engineering and yet became indispensable in this field. This work is concerned with the development of a concept for nonlinear feature selection from scalar, multivariate data on the basis of the mutual information. From the viewpoint of modelling, the successful construction of a realistic model depends highly on the quality of the employed data. In the ideal case, high quality data simply consists of the relevant features for deriving the model. In this context, it is important to possess a suitable method for measuring the degree of the, mostly nonlinear, dependencies between input- and output variables. By means of such a measure, the relevant features could be specifically selected. During the course of this work, it will become evident that the mutual information is a valuable and feasible measure for this task and hence the method of choice for practical applications. Basically and without the claim of being exhaustive, there are two possible constellations that recommend the application of feature selection. On the one hand, feature selection plays an important role, if the computability of a derived system model cannot be guaranteed, due to a multitude of available features. On the other hand, the existence of very few data points with a significant number of features also recommends the employment of feature selection. The latter constellation is closely related to the so called "Curse of Dimensionality". The actual statement behind this is the necessity to reduce the dimensionality to obtain an adequate coverage of the data space. In other word, it is important to reduce the dimensionality of the data, since the coverage of the data space exponentially decreases, for a constant number of data points, with the dimensionality of the available data. In the context of mapping between input- and output space, this goal is ideally reached by selecting only the relevant features from the available data set. The basic idea for this work has its origin in the rather practical field of automotive engineering. It was motivated by the goals of a complex research project in which the nonlinear, dynamic dependencies among a multitude of sensor signals should be identified. The final goal of such activities was to derive so called virtual sensors from identified dependencies among the installed automotive sensors. This enables the real-time computability of the required variable without the expenses of additional hardware. The prospect of doing without additional computing hardware is a strong motive force in particular in automotive engineering. In this context, the major problem was to find a feasible method to capture the linear- as well as the nonlinear dependencies. As mentioned before, the goal of this work is the development of a flexibly applicable system for nonlinear feature selection. The important point here is to guarantee the practicable computability of the developed method even for high dimensional data spaces, which are rather realistic in technical environments. The employed measure for the feature selection process is based on the sophisticated concept of mutual information. The property of the mutual information, regarding its high sensitivity and specificity to linear- and nonlinear statistical dependencies, makes it the method of choice for the development of a highly flexible, nonlinear feature selection framework. In addition to the mere selection of relevant features, the developed framework is also applicable for the nonlinear analysis of the temporal influences of the selected features. Hence, a subsequent dynamic modelling can be performed more efficiently, since the proposed feature selection algorithm additionally provides information about the temporal dependencies between input- and output variables. In contrast to feature extraction techniques, the developed feature selection algorithm in this work has another considerable advantage. In the case of cost intensive measurements, the variables with the highest information content can be selected in a prior feasibility study. Hence, the developed method can also be employed to avoid redundance in the acquired data and thus prevent for additional costs.
The dissertation deals with the general problem of how the brain can establish correspondences between neural patterns stored in different cortical areas. Although an important capability in many cognitive areas like language understanding, abstract reasoning, or motor control, this thesis concentrates on invariant object recognition as application of correspondence finding. One part of the work presents a correspondence-based, neurally plausible system for face recognition. Other parts address the question of visual information routing over several stages by proposing optimal architectures for such routing ('switchyards') and deriving ontogenetic mechanisms for the growth of switchyards. Finally, the idea of multi-stage routing is united with the object recognition system introduced before, making suggestions of how the so far distinct feature-based and correspondence-based approaches to object recognition could be reconciled.
Die vorliegende Arbeit befasst sich mit der übergeordneten Frage nach dem Einfluss der Multikollinearität bei der Spezifikation und Schätzung von latenten nichtlinearen Effekten in Strukturgleichungsmodellen. Da das Multikollinearitätsproblem in latenten nicht-linearen Strukturgleichungsmodellen bisher in der Literatur kaum Beachtung gefunden hat, wird als erstes die Frage beantwortet, welche Relevanz das Vorliegen von Multikollinearität in nichtlinearen Strukturgleichungsmodellen hat. Dabei wird aufgezeigt, dass Multikollinearität von Prädiktoren nicht nur bei linearen Strukturgleichungsmodellen ein Problem darstellt, sondern vor allem bei nicht-linearen Strukturgleichungsmodellen aufgrund des Vorhandenseins nicht-linearer Terme. Da nicht-lineare Terme untereinander in einem höheren Zusammenhang stehen können als lineare Terme eines Strukturmodells, kann es zu Scheineffekten und zu verzerrten Schätzungen der nichtlinearen Effekte kommen. Ein Teststärkeverlust tritt immer ein. Als zweites wird gezeigt, dass die am stärksten vertretene Ansatzklasse zur simultanen Schätzung von Interaktionseffekten und quadratischen Effekten, nämlich die Klasse der Produkt-Indikator-Ansätze, seit ihrer Erstpublikation durch Kenny und Judd (1984) Spezifikationsfehler aufweist, welche in ähnlicher Form auch in jüngeren Publikationen enthalten sind (z.B. bei Lee, Song & Poon, 2004). Spezifikationsfehler machen die ganze Ansatzklasse für Multikollinearität „anfällig“, so dass es theoretisch zu verzerrten Schätzungen oder Scheineffekten kommen muss. Es wird gezeigt, wie man die Spezifikationsfehler korrigieren kann, d.h. wie man die bestehenden Ansätze erweitern muss. Da die latente Prädiktorkorrelation Bestandteil der Spezifikationsfehler ist, stellt sich die Frage, wie groß die Verzerrungen sind, die sich bei der Anwendung der (unkorrigierten) Produkt-Indikator-Ansätze ergeben, wenn die Multikollinearität steigt. Um diese Frage zu beantworten, wird eine Simulationsstudie durchgeführt. Sie zeigt, dass der Zwei-Stufen-Ansatz von Ping (1996) verzerrte Schätzungen der nicht-linearen Effekte produziert. Dieser Ansatz ist ein Vertreter der Produkt-Indikator-Ansätze und enthält die besagten Spezifikationsfehler. In der Simulationsstudie wird außerdem gezeigt, dass die korrekte Erweiterung des Ansatzes von Jöreskog und Yang (1996) zu erwartungstreuen Schätzungen führt. Als drittes wird der Frage nachgegangen werden, welche theoretischen und empirischen Unterschiede zwischen den neuen verteilungsanalytischen Ansätzen LMS (Klein & Moosbrugger, 2000) und QML (Klein & Muthén, 2007), einerseits, und den klassischen Produkt-Indikator-Ansätzen, andererseits, zu erwarten sind. Dazu werden die theoretischen Eigenschaften der verteilungsanalytischen Verfahren, LMS und QML, untereinander und mit der Klasse der Produkt-Indikator-Ansätze verglichen. Ihre theoretischen Eigenschaften werden beim Vorliegen von Multikollinearität betrachtet. Illustriert werden die theoretischen Unterschiede anhand einer Simulationsstudie, die die Ansätze hinsichtlich ihrer Schätzeigenschaften bei steigender Multikollinearität miteinander vergleicht. In der Simulationsstudie werden die theoretisch herausgearbeiteten Vorteile der verteilungsanalytischen Verfahren, LMS und QML, bestätigt. In der Zusammenschau der theoretischen Überlegungen und der Ergebnisse aus den Simulationsstudien zeigt sich, dass Multikollinearität für alle Ansätze zur simultanen Schätzung multipler nicht-linearer Effekte eine Herausforderung ist. Die forschungspraktische und inhaltliche Relevanz des Multikollinearitätsproblems wird anhand eines Beispiels veranschaulicht. Für in jüngster Zeit diskutierte komplexere Modelle (z.B. moderierte Mediatormodelle) wird Multikollinearität ein Kernproblem sein, dem sich alle Ansätze werden stellen müssen oder für das neue Ansätze entwickelt werden müssen.
Prognostische Bedeutung der R1 Resektion : verbessertes Überleben in der Ära elektiver Chemotherapie

Einleitung: Die Resektion kolorektaler Lebermetastasen ist der alleinigen Chemotherapie überlegen und erfolgt unter kurativer Zielsetzung. Multimodale Therapiekonzepte erlauben zunehmend primär irresektable Metastasen sekundär zu resezieren. Bei einem Teil der Patienten wird das Ziel der Tumorfreiheit nicht erreicht. Anhand einer retrospektiven Analyse wurde der Anteil der nicht kurativ resezierten Patienten nach primärer und sekundärer Resektion und ihr Überleben bestimmt. Material und Methoden: Bei 152 Patienten wurden zwischen 1/02 – 7/07 insgesamt 174 Leberresektionen aufgrund kolorektaler Metastasen durchgeführt. Dieses Kollektiv wurde anhand des R-Status der Leberresektion in eine kurativ- (Gruppe I) und in eine nicht kurativ resezierte Gruppe (Gruppe II) unterteilt. Die Auswertung erfolgte retrospektiv nach Überleben, Metastasenausdehnung, neoadjuvanter Chemotherapie und primärem Tumorstadium. Ergebnisse: 174 Leberresektionen schlossen 44 Re-Resektion und 6 zweizeitige Resektionen ein. Bei 61 Patienten erfolgte eine neoadjuvante Chemotherapie vor der Leberresektion. Die demographischen Daten beider Gruppen zeigten keinen signifikanten Unterschied (Gr. I: Männer: 60%, Alter: 63+/-1; Gr. II: Männer: 65%, Alter: 57+/-2,4). Die Anzahl der resezierten Metastasen war in Gruppe II signifikant höher (Gr. I: 1,5+/-0,1; Gr. II: 3,5+/-0,7, p<0,01), während die lokale Primärtumor-Ausdehnung gemessen am T-Stadium (Gr. I: 2,8 +/- 0,1, Gr. II: 2,8 +/- 0,13, n.s.) keinen Unterschied zeigte. Der Anteil der nicht kurativ resezierten Patienten war nach primärer und sekundärer Resektion gleich (17% vs. 21%, n.s.). Das Gesamtüberleben (4-Jahre, Kaplan-Meier) (Gr. I: 61%, Gr. II: 29%, p<0,05) und das mediane Überleben (Gr. I: 4,4, Gr. II: 2,4 [Jahre], p<0,05) wiesen einen signifikanten Unterschied auf. Schlussfolgerung: Der R-Status nach Resektion kolorektaler Lebermetastasen ist weiterhin ein entscheidender prognostischer Faktor. R-1 resezierte Patienten erreichen jedoch mit 2,4 Jahren ein besseres medianes Überleben als vergleichbare historische Kollektive. Sekundäre Resektionen nach neoadjuvanter Therapie weisen eine vergleichbar hohe Rate an R1 Resektionen auf wie bei primären Resektionen.
New histone deacetylase inhibitors as potential therapeutic tools for advanced prostate carcinoma

The anti-epileptic drug valproic acid is also under trial as an anti-cancer agent due to its histone deacetylase (HDAC) inhibitory properties. However, the effects of valproic acid (VPA) are limited and concentrations required for exerting anti-neoplastic effects in vitro may not be reached in tumour patients. In this study, we tested in vitro and in vivo effects of two VPA-derivatives (ACS2, ACS33) on pre-clinical prostate cancer models. PC3 and DU-145 prostate tumour cell lines were treated with various concentrations of ACS2 or ACS33 to perform in vitro cell proliferation 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assays and to evaluate tumour cell adhesion to endothelial cell monolayers. Analysis of acetylated histones H3 and H4 protein expression was performed by western blotting. In vivo tumour growth was conducted in subcutaneous xenograft mouse models. Tumour sections were assessed by immunohistochemistry for histone H3 acetylation and proliferation. ACS2 and ACS33 significantly up-regulated histone H3 and H4 acetylation in prostate cancer cell lines. In micromolar concentrations both compounds exerted growth arrest in PC3 and DU-145 cells and prevented tumour cell attachment to endothelium. In vivo, ACS33 inhibited the growth of PC3 in subcutaneous xenografts. Immunohistochemistry and western blotting confirmed increased histone H3 acetylation and reduced proliferation. ACS2 and ACS33 represent novel VPA derivatives with superior anti-tumoural activities, compared to the mother compound. This investigation lends support to the clinical testing of ACS2 or ACS33 for the treatment of prostate cancer.
Interleukin (IL)-18, formerly known as interferon (IFN)-γ-inducing factor, is a crucial mediator of host defence and inflammation. Control of IL-18 bioactivity by its endogenous antagonist IL-18 binding protein (IL-18BP) is a major objective of immunoregulation. IL-18BP is strongly up-regulated by IFN-γ, thereby establishing a negative feedback mechanism detectable in cell culture and in vivo. Here we sought to investigate in D.L. Dexter (DLD) colon carcinoma cells molecular mechanisms of IL-18BP induction under the influence of IFN-γ. Mutational analysis revealed that a proximal γ-activated sequence (GAS) at the IL-18BP promoter is of pivotal importance for expression by IFN-γ-activated cells. Use of siRNA underscored the essential role of the signal transducer and activator of transcription (STAT)-1 in this process. Indeed, electrophoretic mobility shift assay and chromatin immunoprecipitation analysis proved STAT1 binding to this particular GAS site. Maximal expression of IL-18BP was dependent on de novo protein synthesis but unaffected by silencing of interferon regulatory factor-1. Altogether, data presented herein indicate that direct action of STAT1 on the IL-18BP promoter at the proximal GAS element is key to IL-18BP expression by IFN-γ-stimulated DLD-1 colon carcinoma cells.
Obwohl zur Diagnostik kognitiver Funktionen bei Patienten mit psychischen Störungen computerisierte neuropsychologische Leistungstests inzwischen das Mittel der Wahl sind, gibt es kaum Untersuchungen, ob es eine besondere Patient-Computer-Interaktion gibt, und ob diese Interaktion Einfluss auf die Ergebnisse neuropsychologischer Leistungstests hat. Aufgrund der spezifischen Psychopathologie depressiver Erkrankungen ist diese Fragestellung hierzu besonders relevant. In der vorliegenden Studie wurde deshalb die Patient-Computer-Interaktion zunächst mit den Variablen Computererfahrung, Computereinstellung und Wahrnehmung der Testsituation operationalisiert. Anschließend wurde geprüft, ob sich depressive Patienten in diesen Variablen von Kontrollpersonen unterscheiden, ob die Computererfahrung und die Computereinstellung jeweils einen Einfluss auf die Ergebnisse von neuropsychologischen Leistungstests haben und ob diese Einflüsse besonders stark bei depressiven Patienten zum Tragen kommen. Es wurden 73 Patienten mit rezidivierend depressiver Störung mit 73 Alters und Geschlecht gematchten Kontrollpersonen verglichen. Die Diagnose erfolgte anhand der Internationalen Diagnose Checklisten nach DSM IV. Zur Erfassung der Patient-Computer-Interaktion wurden der Demographische Fragebogen zu Umgang mit Computern, die Groningen Computer Attitude Scale sowie ein neu entwickelter Fragebogen zur Wahrnehmung der Testsituation eingesetzt. Die umfangreiche neuropsychologische Untersuchung erfolgte mit den Testbatterien NEUROBAT und CANTAB sowie einer Visuomotorischen Tracking-Aufgabe. Depressive Patienten zeigten kognitive Leistungsdefizite gegenüber Kontrollpersonen in einer Gedächtnis Wiedererkennen-, einer Visuomotorischen Tracking- und in einer Arbeitsgedächtnisaufgabe. Die Patienten hatten weniger Computererfahrung, eine weniger positive Computereinstellung und nahmen die Testsituation weniger positiv wahr als Kontrollpersonen, obwohl die Computereinstellung und die Wahrnehmung der Testsituation auch bei den Patienten überwiegend positiv war. Sowohl Patienten als auch Kontrollpersonen mit geringer Computererfahrung oder negativerer Computereinstellung zeigten in einigen neuropsychologischen Tests schlechtere Ergebnisse trotz der statistischen Kontrolle möglicher konfundierender Variablen wie Alter, Geschlecht und Bildung. Die Computererfahrung fungierte dabei von allen Variablen als häufigster Prädiktor zu Erklärung eines Testergebnisses. Depressive Patienten mit negativerer Computereinstellung reagierten langsamer bei den Tests der NEUROBAT-Testbatterie. Ansonsten ergaben sich keine weiteren Interaktionen Gruppe x Computererfahrung bzw. Gruppe x Computereinstellung. Computerisierte neuropsychologische Leistungstests werden von Patienten mit rezidivierend depressiver Störung überwiegend positiv wahrgenommen. Sowohl bei den Patienten als auch bei den Kontrollpersonen haben die Computererfahrung und Computereinstellung stellenweise einen Einfluss auf die Testergebnisse. Die Nichtberücksichtigung dieser Variablen kann somit zur Benachteiligung von Personen führen, die über geringe Computererfahrung oder eine negativere Computereinstellung verfügen. Rezidivierend depressive Patienten sind bei Nichtberücksichtigung dieser Variablen nicht aufgrund ihrer spezifischen Psychopathologie benachteiligt, sondern wegen ihrer geringeren Computererfahrung und negativeren Computereinstellung. Bei der Untersuchung kognitiver Funktionen mit computerisierten neuropsychologischen Leistungstests sollten deshalb zukünftig sowohl bei wissenschaftlichen als auch bei klinischen Fragestellungen immer die Computererfahrung und die Computereinstellung mitberücksichtigt werden.
