Einführung: Die höchste Offenheitsrate in der arteriellen Bypasschirurgie der unteren Extremitäten wird mit einem Segment der Vena saphena magna (VSM) von ausreichender Länge und Durchmesser erreicht. Dabei ist der Venendurchmesser der einzige signifikant korrelierende Faktor für das Auftreten einer Transplantatsstenose und damit der beste Prädiktor für die primäre und die sekundäre 2-Jahre-Durchgängigkeitsrates. In 20-40% der Patienten fehlt ein bzgl. des Lumens geeignetes Segment der Vena saphena magna zur Herstellung eines Bypasses bei der primären Operation. Daher wurde eine Technik entwickelt, kleinlumige Vena saphena magna Segmente mittels einer Valvulotomie zu erweitern.
Ziel: Eine zu kleinlumige Vena saphena magna soll VSM für eine spätere Verwendung als autologes Bypassmaterial der unteren Extremität konditioniert werden. Die häufigsten Indikationen für eine derartige Konditionierung sind die Bypassverwendung bei der operativen Therapie der peripheren arteriellen Verschlusskrankheit (pAVK) und beim Aneurysma der Arteria poplitea (PA).
Methoden: Diese prospektive Pilotstudie würde zwischen Juni 2007 und November 2011 durchgeführt. 25 Patienten (26 Fälle) mit einem VSM Durchmesser zwischen 2 und 3mm erhielten eine in situ VSM-Valvulotomie. Die Zerstörung der suffizienten Venenklappen ermöglicht einen retrograden Blutstrom in die Vena saphena magna. Der hierdurch erzeugte Wachstumsreiz soll zu einer Durchmesserzunahme der Vene führen. Nach Erreichen eines Durchmessers von > 3mm für infragenuale Rekonstruktionen und von > 3,5 mm für supragenuale Rekonstruktion wurden die konditionierten Venensegmente als Bypass implantiert. Bei 23 Patienten war die Bypassindikation eine nicht kritische pAVK. Bei 2 Patienten war ein Aneurysma der Arteria poplitea die Indikation. Einschlusskriterium für Teilnahme an der Studie war eine nicht variköse VSM mit einem Querdurchmesser zwischen 2 und 3 mm. Die Venenevaluation erfolgte duplexsonografisch im Stehen durch eine Beurteilung der epifaszialen Vena saphena magna von der Leiste bis zum Innenknöchel. Messpunkte waren: 10 cm unterhalb des Hiatus saphenus, 10 cm oberhalb des Knies, 10 cm unterhalb des Knies und 10 cm oberhalb des Innenknöchels. Um das Venenwachstum zu ermitteln, diente als Messpunkt der kleinste ermittelte Durchmesser über einem Messbereich von 10 mm.
Drei Patienten, bei denen ein femoropoplitealer Bypass oberhalb des Kniegelenkes implantiert wurde, hatten bei der Implantation einen Durchmesser der konditionierten VSM > 3.5 mm. Bei 13 Patienten mit Bypassimplantation unterhalb des Kniegelenkes war die konditionierte VSM > 3mm. Eine Wiederholung der Valvulotomie war in keinem der Patienten nötwendig. Die valvulotomierte VSM wurde bei allen Bypassoperationen offen chirurgisch entnommen. Zur postoperativen Kontrolle erfolgten eine Duplexuntersuchung und eine ABI-Messung 1,3,6,12,18 und 24 Monate nach der Implantation.
Ergebnisse: Durchschnittlich vergrößerte sich der VSM-Durchmesser von initial 2,5±0,18 mm vor Valvulotomie auf 2,8±0,2 mm, 3,0±0,3mm und 3,2±0,4 mm jeweils 30, 60 und 90 Tage nach Valvolutomie. Bei der Entnahme betrug der durchschnittliche Bypassdurchmesser 3.7±0.6 mm. Die primäre 1-Jahres-Offenheitsrate mit der valvulotomierten VSM war 81±9,8% im Vergleich zu in der Literatur publizierten 58,0%±8,4% für alloplastische Transplantate, 51% für kleinkalibrige VSM und 81,6%±3,6% für Armvenentransplantate. Die sekundäre 1-Jahres-Offenheitsrate mit der Valvulotomietechnik betrug 87%±8,3% verglichen mit 82,6% für in der Literatur angegebene Transplantationen mit kleinkalibriger VSM. Die primäre 2-Jahres-Offenheitsrate mit der Valvulotomietechnik betrug 69%±11,8%, verglichen mit 50% in der Literatur angegebener Transplantationen von kleinkalibrige VSM benutzt wurden und 72%, falls Armvenentransplantate benutzt wurden. Die sekundäre 2-Jahres-Offenheitsrate mit der Valvulotomietechnik lag bei 75%±11%.
Fazit: Die Valvulotomie kann zur Zunahme des Venendurchmessers vor Anlage eines femoro-distalen Bypasses verwendet werden, weil sie regelhaft zum Reflux in der valvulotomierten Vene führt und dadurch ein relevantes Venenwachstum verursacht. Zwischen 60 und 90 Tagen nach Valvulotomie wird ein Venenduchmesser > 3.5 mm erreicht und die valvulotomierte VSM kann als femoropoplitealer Bypass oberhalb des Kniegelenkes implantiert werden. Für eine Anastomose unterhalb des Kniegelenkes genügt ein Wachstum bis > 3 mm. Wenn die valvulotomierte Vene einen entsprechenden Durchmesser erreicht, kann die konditionierte VSM benutzt werden, um einen Bypass mit guter Offenheitsprognose zu konstruieren oder einen nicht funktionierenden Bypass zu ersetzen.
Einleitung: Die Intention dieser Arbeit war es zu evaluieren, ob das Prüfungsformat eines OSPE (Objective Structured Practical Examination), durchgeführt im Fach Zahnerhaltungskunde (6. Fachsemester), Aussagen über den Studienerfolg im praktischen Teil des Staatsexamens (11. Fachsemester) im selben Fach zu treffen erlaubt. Ferner sollte unter Berücksichtigung allgemeiner Angaben der StudienteilnehmerInnen (Abitursnote, Physikumsnote, Studiendauer, Kohorte und Geschlecht) analysiert werden, ob bezüglich der Gesamt- sowie Teilnoten des OSPE und der Staatsexamensprüfung im Fach Zahnerhaltungskunde Zusammenhänge oder Unterschiede bestehen.
Methoden: Im Rahmen dieser longitudinalen, retrospektiven Studie wurden über einen Zeitraum von 11 Semestern prüfungsbezogene Daten von Studierenden (N = 223) des Fachbereichs Zahnmedizin in Frankfurt am Main erhoben und untersucht. Ferner wurden persönliche Angaben der Studienteilnehmer zu Alter, Geschlecht, Abitursnote, Physikumsnote und Studiendauer evaluiert sowie eine Zuordnung zu Kohorten vorgenommen. Für die statistische Auswertung der Daten wurden Spearman Rangkorrelationen, Partialkorrelationen, Korrelationskoeffizienten nach Pearson, und Multiple Regressionen (SPSS Statistics 21, IBM Corporation, New York) berechnet.
Ergebnisse: Die Ergebnisse zeigen, dass die Durchführung einer OSPE (Cronbachs α = .87) mit dem Erfolg im praktischen Teil des Staatsexamens im Fach Zahnerhaltungskunde korreliert (p = .01, r = .17). Als eine weitere signifikante Korrelation mit der Examensleistung erwies sich die Dauer des Studiums (p = .001, r = .23). Gemeinsam vermögen diese beiden Variablen einen signifikanten Beitrag zur Vorhersage der Examensnote (p = .001, R2 = .076) zu leisten. Das zeigte sich im größeren Umfang bei weiblichen Studierenden. Zudem wurde festgestellt, dass diese bessere Abitursnoten als männliche Studierende aufweisen (F = 6.09, p = .01, η² = .027) und dass es nur bei männlichen Studierenden eine signifikante Korrelation zwischen der Physikumsnote (Zahnärztliche Vorprüfung) und der OSPE-Benotung gab (r = .17, p = .01).
Schlussfolgerung: In der vorliegenden Untersuchung konnte der prädiktive Effekt eines durchgeführten klinischen OSPE auf die Prüfungsleistung im Staatsexamen nachgewiesen werden. Unter Berücksichtigung der Limitation der Studie empfiehlt sich aus unserer Sicht die Durchführung eines solchen Prüfungsformats im Rahmen des klinischen Studienabschnitts im 6. Semester im Fach Zahnmedizin.
Ursprünglich sollte das vorliegende Buch nur eine Studie über die von der Kanzlei des Führers bzw. ihrem Umfeld nach Kriegsbeginn initiierten Entwürfe zu einem NS-„Euthanasie“-Gesetz werden. Dabei sollte vor allem der wichtige, allerdings mit zahlreichen Fehlern behaftete Beitrag von Roth/Aly(1984) revidiert werden. Da diese Fehler auch den Beginn der Debatte über die „Euthanasie“ im NS-Staat und den Beginn der Planung und Durchführung der „NS-Euthanasie“ (1939/1940) betrafen, musste weiter ausgeholt werden. Die Gesetzentwürfe aus der Zeit nach Kriegsbeginn machen unzweifelhaft deutlich, dass die am Krankenmord Beteiligten wussten, dass der auf den 1.9.1939 datierte „Euthanasie“-Erlass Hitlers in legaler Hinsicht nicht „ausreichte“.
Mit flexiblen Video-Endoskopen gelingen heute hochaufgelöste Bilder des Magen-Darm-Traktes. Bösartige Tumoren werden früher erkannt und oft auch entfernt, ohne die Bauchdecke aufzuschneiden. Sogar Verengungen der Gallenwege lassen sich mit hochpräziser Endoskopietechnik darstellen und behandeln. Die Medizinische Klinik 1 der Universitätsklinik unter der Leitung von Prof. Dr. Stefan Zeuzem gehört zu den Pionieren auf diesem Gebiet.
Bestimmung des klinischen Nutzens systemischer adjuvanter Therapien beim frühen Mammakarzinom

Die onkologische Therapie befindet sich im Umbruch. Hohe Erwartungen sind mit einer Reihe innovativer zielgerichteter Medikamente verknüpft, die sich derzeit in der klinischen Entwicklung befinden. Vor diesem Hintergrund erfahren Diskussionen um die Begriffe klinischer Nutzen oder klinische Relevanz neue Aktualität. Dies gilt auch für die Weiterentwicklungen der adjuvanten systemischen Therapie des frühen Mammakarzinoms. In Anbetracht der kurativen Zielsetzung erfolgt die Beurteilung des klinischen Nutzens einer adjuvanten Therapie maßgeblich anhand von Wirksamkeitsendpunkten. Der Fokus liegt hierbei auf Verbesserungen des krankheitsfreien Überlebens und des Rezidivrisikos. Eine Aussage zum Gesamtüberleben ist aufgrund der heute erreichten niedrigen Mortalitätsraten erst nach sehr langen Beobachtungszeiten möglich. Folgerichtig sollte neuen Medikamenten für die adjuvante Therapie ein klinischer Nutzen zugesprochen werden, wenn sie eine weitere Reduktion des Rezidivrisikos über den heutigen hohen Standard hinaus ermöglichen. Die Evidenz für etablierte adjuvante Therapiestandards beim frühen Mammakarzinom kann als objektiver Maßstab zum Vergleich herangezogen werden. Am Beispiel der adjuvanten endokrinen Therapie, der adjuvanten Polychemotherapie und der adjuvanten Anti-HER2-Therapie werden in diesem Übersichtsartikel die Anforderungen für den klinischen Nutzen neuer adjuvanter Therapien beim frühen Mammakarzinom abgeleitet.
Oncologic therapy is currently undergoing significant changes. A number of innovative targeted medications currently in clinical development have raised high expectations. With that in mind, discussions about terms such as "clinical benefit" and "clinical relevance" are highly topical. This also applies to further developments in the field of adjuvant systemic therapies for early-stage breast cancer. As the treatment aim is curative, assessment of the clinical benefit of adjuvant therapies must be largely based on efficacy outcomes. The focus must be on improving disease-free survival rates and lowering the risk of recurrence. Because of the current low mortality rates, statements about overall survival rates are only possible after very long observation periods. Consequently, new drugs in adjuvant therapies should be considered as offering a clinical benefit, if they reduce the risk of recurrence below current low levels of risk. The evidence for established adjuvant therapy standards in early-stage breast cancer can be used as objective criteria for comparison. This review article considers the requirements for clinical benefit of new adjuvant therapies for early breast cancer, based on examples from adjuvant endocrine therapy, adjuvant polychemotherapy and adjuvant anti-HER2 therapy.
Kurt Goldstein zählt zu den herausragenden deutschen Neurologen des 20. Jahrhunderts. Bekannt wurde er u. a. durch einen eigenständigen Ansatz der ganzheitlichen Neurologie. Besondere Aufmerksamkeit schenkte er dabei den Kompensationsreaktionen des Gehirns und des ganzen Menschen. In der vorliegenden Studie wird erstmals der Lebensgang dieses jüdischen Arztes in den Jahren 1933 bis 1940 genauer betrachtet.
The CRISPR/Cas9 prokaryotic adaptive immune system and its swift repurposing for genome editing enables modification of any prespecified genomic sequence with unprecedented accuracy and efficiency, including targeted gene repair. We used the CRISPR/Cas9 system for targeted repair of patient-specific point mutations in the Cytochrome b-245 heavy chain gene (CYBB), whose inactivation causes chronic granulomatous disease (XCGD)—a life-threatening immunodeficiency disorder characterized by the inability of neutrophils and macrophages to produce microbicidal reactive oxygen species (ROS). We show that frameshift mutations can be effectively repaired in hematopoietic cells by non-integrating lentiviral vectors carrying RNA-guided Cas9 endonucleases (RGNs). Because about 25% of most inherited blood disorders are caused by frameshift mutations, our results suggest that up to a quarter of all patients suffering from monogenic blood disorders could benefit from gene therapy employing personalized, donor template-free RGNs.

Autophagy in cancer therapy

Autophagy represents a catabolic program involved in the degradation of cellular components via lysosomes. It serves to mitigate cellular stress and to provide metabolic precursors especially upon starvation. Thereby, autophagy can support the survival of cancer cells. In addition, there is now convincing evidence showing that under certain conditions autophagy can also foster cell death. This dual function of autophagy is also relevant upon anticancer treatment, as many chemotherapeutic agents engage autophagy. A better understanding of the molecular mechanisms that are critical for mediating autophagic cell death in cancer cells will be instrumental to selectively interfere with this cellular program in order to increase the cancer cell’s response to cytotoxic drugs. This review illustrates how anticancer drug-induced autophagy is involved in mediating cell death.
Characterization of a novel KCNJ2 sequence variant detected in Andersen-Tawil syndrome patients

Background: Mutations in the KCNJ2 gene encoding the ion channel Kir2.1 have been linked to the Andersen-Tawil syndrome (ATS). Molecular genetic screening performed in a family exhibiting clinical ATS phenotypes unmasked a novel sequence variant (c.434A > G, p.Y145C) in this gene. The aim of this study was to investigate the effect of this variant on Kir2.1 ion channel functionality.
Methods: Mutant as well as wild type GFP tagged Kir2.1 channels were expressed in HEK293 cells. In order to examine the effect of the new variant, electrophysiological measurements were performed using patch clamp technique. Cellular localization of the mutant in comparison to the wild type ion channel was analyzed by confocal laser scanning microscopy.
Results: The currents of cells expressing only mutant channels or a mixture of wild type and mutant were significantly reduced compared to those expressing wild type (WT) channels (p < 0.01). Whereas WT expressing cells exhibited at −120 mV an averaged current of −4.5 ± 1.9 nA, the mutant generates only a current of −0.17 ± 0.07 nA. A co-expression of mutant and WT channel generates only a partial rescue of the WT current. Confocal laser scanning microscopy indicated that the novel variant is not interfering with synthesis and/or protein trafficking.
Conclusions: The detected sequence variant causes loss-of-function of the Kir2.1 channel and explains the clinical phenotypes observed in Andersen-Tawil syndrome patients.

As new generations of targeted therapies emerge and tumor genome sequencing discovers increasingly comprehensive mutation repertoires, the functional relationships of mutations to tumor phenotypes remain largely unknown. Here, we measured ex vivo sensitivity of 246 blood cancers to 63 drugs alongside genome, transcriptome, and DNA methylome analysis to understand determinants of drug response. We assembled a primary blood cancer cell encyclopedia data set that revealed disease-specific sensitivities for each cancer. Within chronic lymphocytic leukemia (CLL), responses to 62% of drugs were associated with 2 or more mutations, and linked the B cell receptor (BCR) pathway to trisomy 12, an important driver of CLL. Based on drug responses, the disease could be organized into phenotypic subgroups characterized by exploitable dependencies on BCR, mTOR, or MEK signaling and associated with mutations, gene expression, and DNA methylation. Fourteen percent of CLLs were driven by mTOR signaling in a non–BCR-dependent manner. Multivariate modeling revealed immunoglobulin heavy chain variable gene (IGHV) mutation status and trisomy 12 as the most important modulators of response to kinase inhibitors in CLL. Ex vivo drug responses were associated with outcome. This study overcomes the perception that most mutations do not influence drug response of cancer, and points to an updated approach to understanding tumor biology, with implications for biomarker discovery and cancer care.
Die Herztransplantation ist die Goldstandardtherapie der Herzinsuffizienz [24]. Verglichen mit der konventionellen Therapie führt sie zu einer verbesserten Überlebensrate, Belastbarkeit und Lebensqualität [15b]. Diese werden durch die internationale Gesellschaft für Herz- und Lungentransplantation (ISHLT) mit ihrer weltweit größten Datenbank für Herz- und Lungentransplantationsdaten überprüft [45]. Aufgrund des hohen Stellenwerts und der umfassenden Arbeit der ISHLT findet in dieser Dissertation der Vergleich der Daten über das Outcome von 76 Herztransplantierten der Johann-Wolfgang-Goethe-Universitätsklinik Frankfurt am Main (KGU) mit den Daten der ISHLT statt.
Es handelt sich bei dieser Arbeit um eine retrospektive Studie, die 60 Männer und 16 Frauen im Alter zwischen 18 und 68 Jahren einschließt, die an der Uniklinik Frankfurt zwischen dem 24.02.1997 und dem 04.05.2013 eine Herztransplantation erhielten. Das Ziel dieser Arbeit ist es, die Ergebnisse der Universitätsklinik Frankfurt a.M. mit den internationalen Werten zu vergleichen um genauere Informationen zur internationalen Positionierung zu erhalten. Die Resultate sollen dazu dienen, die eigenen Prozesse zu verbessern, Stärken auszubauen und Schwächen zu beheben und die Behandlung der Patienten den wissenschaftlichen Empfehlungen anzupassen.
Die Ergebnisse dieser Studie zeigten, dass die 1-Jahres-Überlebenswahrscheinlichkeit an der KGU mit 69% deutlich geringer war als international mit 81% [37a]. Dabei zeigte sich in genaueren Analysen, dass das schlechtere Kurzzeitüberleben vor allem von der Transplantationsära von Februar 1997 bis 2001, der zur Transplantation führenden Diagnose einer ischämischen Kardiomyopathie (ICM) und dem Vorhandensein eines Assist Devices vor der Transplantation verursacht wurde. Das Alter und das Geschlecht des Patienten dagegen schienen keinen Einfluss darauf zu nehmen. Im Langzeitüberleben lagen die Ergebnisse der KGU deutlich über den Werten der ISHLT. Unter der Bedingung, dass das erste postoperative Jahr überlebt wurde, ergab sich für KGU Patienten eine 10- bzw. 15-Jahres-Überlebenswahrscheinlichkeit von 74% (n=40) bzw. 60% (n=33). Die Wahrscheinlichkeit das 10. postoperative Jahr zu überleben lag für ISHLT Patienten bei 63,9%; für das 15. Jahr lag dieser Wert bei 42% [37g].
Die Langzeitkomplikationen der Malignität und der TVP traten an der KGU anteilig seltener auf als international. Die TVP-Freiheit nach 10 Jahren lag international bei 47% und an der KGU bei 75% [37e]. Für die Freiheit von Malignität lag die Wahrscheinlichkeit des 10. Jahres international bei 66% und an der KGU bei 74% [37c]. Die Wahrscheinlichkeit für die Freiheit von einem Kreatininwert über 2,5mg/dl bis zum 10. Jahr nach Transplantation war an der KGU mit 70% (n=26) ähnlich wie die internationalen Werte von 61,5% [37b]. Die Patienten der KGU erlitten des Weiteren deutlich mehr Abstoßungen im ersten Jahr. Während international zwischen 25 und 30% der Patienten eine Abstoßungsreaktion im ersten Jahr erlebten, waren es an der KGU mindestens 50%. Trotzdem war der Anteil an behandlungsbedürftigen Abstoßungen an der KGU anteilig geringer als bei dem Patientenkollektiv der ISHLT [37d]. Bei der Analyse der Immunsuppression zeigte sich, dass die KGU das Steroidweaning deutlich später und in wesentlich geringerem Ausmaß durchführt: während im 5. postoperativen Jahr 48% der ISHLT-Patienten Steroide erhalten, sind es an der KGU noch 83% (n=40) [37n]. Im Vergleich der verschiedenen Kombinationstherapien zeigte sich, dass in der Patientengruppe der KGU hauptsächlich Kombinationen mit Ciclosporin (Jahr 1 85,2%, n=46) angewendet wurden, während unter den ISHLT-Patienten hauptsächlich Tacrolimus (Jahr 1 57,5%, n=4053) eingesetzt wurde. Beide Gruppen haben gemeinsam, dass der Anteil an Sirolimus- bzw. Everolimus-Kombinationen von Jahr 1 auf Jahr 5 zunimmt. Dabei kommen diese Kombinationen prozentual gesehen international häufiger vor [37m].
Folglich konnte diese Studie nicht nur zeigen, wo die Stärken der Universitätsklinik Frankfurt liegen, sondern auch welche Punkte in naher Zukunft verbessert werden können. Somit werden konkrete Anpassungen an der Behandlung der HTx-Patienten der KGU erfolgen können.
Background: Previous magnetic resonance imaging (MRI) research suggests that, prior to the onset of psychosis, high risk youths already exhibit brain abnormalities similar to those present in patients with schizophrenia.
Objectives: The goal of the present study was to describe the functional organization of endogenous activation in young adolescents who report auditory verbal hallucinations (AVH) in view of the “distributed network” hypothesis of psychosis. We recruited 20 young people aged 13–16 years who reported AVHs and 20 healthy controls matched for age, gender and handedness from local schools.
Methods: Each participant underwent a semi-structured clinical interview and a resting state (RS) neuroimaging protocol. We explored functional connectivity (FC) involving three different networks: 1) default mode network (DMN) 2) salience network (SN) and 3) central executive network (CEN). In line with previous findings on the role of the auditory cortex in AVHs as reported by young adolescents, we also investigated FC anomalies involving both the primary and secondary auditory cortices (A1 and A2, respectively).
Further, we explored between-group inter-hemispheric FC differences (laterality) for both A1 and A2. Compared to the healthy control group, the AVH group exhibited FC differences in all three networks investigated. Moreover, FC anomalies were found in a neural network including both A1 and A2. The laterality analysis revealed no between-group, inter-hemispheric differences.
Conclusions: The present study suggests that young adolescents with subclinical psychotic symptoms exhibit functional connectivity anomalies directly and indirectly involving the DMN, SN, CEN and also a neural network including both primary and secondary auditory cortical regions.
Nepal is highly vulnerable to global climate change, despite its negligible emission of global greenhouse gases. The vulnerable climate-sensitive sectors identified in Nepal's National Adaptation Programme of Action (NAPA) to Climate Change 2010 include agriculture, forestry, water, energy, public health, urbanization and infrastructure, and climate-induced disasters. In addition, analyses carried out as part of the NAPA process have indicated that the impacts of climate change in Nepal are not gender neutral. Vector-borne diseases, diarrhoeal diseases including cholera, malnutrition, cardiorespiratory diseases, psychological stress, and health effects and injuries related to extreme weather are major climate-sensitive health risks in the country. In recent years, research has been done in Nepal in order to understand the changing epidemiology of diseases and generate evidence for decision-making. Based on this evidence, the experience of programme managers, and regular surveillance data, the Government of Nepal has mainstreamed issues related to climate change in development plans, policies and programmes. In particular, the Government of Nepal has addressed climate-sensitive health risks. In addition to the NAPA report, several policy documents have been launched, including the Climate Change Policy 2011; the Nepal Health Sector Programme – Implementation Plan II (NHSP-IP 2) 2010–2015; the National Health Policy 2014; the National Health Sector Strategy 2015–2020 and its implementation plan (2016–2021); and the Health National Adaptation Plan (H-NAP): climate change and health strategy and action plan (2016–2020). However, the translation of these policies and plans of action into tangible action on the ground is still in its infancy in Nepal. Despite this, the health sector's response to addressing the impact of climate change in Nepal may be taken as a good example for other low- and middle-income countries.
Both hemispheres contribute to motor control beyond the innervation of the contralateral alpha motoneurons. The left hemisphere has been associated with higher-order aspects of motor control like sequencing and temporal processing, the right hemisphere with the transformation of visual information to guide movements in space. In the visuomotor context, empirical evidence regarding the latter has been limited though the right hemisphere’s specialization for visuospatial processing is well-documented in perceptual tasks. This study operationalized temporal and spatial processing demands during visuomotor processing and investigated hemispheric asymmetries in neural activation during the unimanual control of a visual cursor by grip force. Functional asymmetries were investigated separately for visuomotor planning and online control during functional magnetic resonance imaging in 19 young, healthy, right-handed participants. The expected cursor movement was coded with different visual trajectories. During planning when spatial processing demands predominated, activity was right-lateralized in a hand-independent manner in the inferior temporal lobe, occipito-parietal border, and ventral premotor cortex. When temporal processing demands overweighed spatial demands, BOLD responses during planning were left-lateralized in the temporo-parietal junction. During online control of the cursor, right lateralization was not observed. Instead, left lateralization occurred in the intraparietal sulcus. Our results identify movement phase and spatiotemporal demands as important determinants of dynamic hemispheric asymmetries during visuomotor processing. We suggest that, within a bilateral visuomotor network, the right hemisphere exhibits a processing preference for planning global spatial movement features whereas the left hemisphere preferentially times local features of visual movement trajectories and adjusts movement online.
Functional imaging studies using BOLD contrasts have consistently reported activation of the supplementary motor area (SMA) both during motor and internal timing tasks. Opposing findings, however, have been shown for the modulation of beta oscillations in the SMA. While movement suppresses beta oscillations in the SMA, motor and non-motor tasks that rely on internal timing increase the amplitude of beta oscillations in the SMA. These independent observations suggest that the relationship between beta oscillations and BOLD activation is more complex than previously thought. Here we set out to investigate this rapport by examining beta oscillations in the SMA during movement with varying degrees of internal timing demands. In a simultaneous EEG-fMRI experiment, 20 healthy right-handed subjects performed an auditory-paced finger-tapping task. Internal timing was operationalized by including conditions with taps on every fourth auditory beat, which necessitates generation of a slow internal rhythm, while tapping to every auditory beat reflected simple auditory-motor synchronization. In the SMA, BOLD activity increased and power in both the low and the high beta band decreased expectedly during each condition compared to baseline. Internal timing was associated with a reduced desynchronization of low beta oscillations compared to conditions without internal timing demands. In parallel with this relative beta power increase, internal timing activated the SMA more strongly in terms of BOLD. This documents a task-dependent non-linear relationship between BOLD and beta-oscillations in the SMA. We discuss different roles of beta synchronization and desynchronization in active processing within the same cortical region.
In this study, we aimed to comparatively evaluate high-resolution 3D ultrasonography (hrUS), in-vivo micro-CT (μCT) and 9.4T MRI for the monitoring of tumor growth in an orthotopic renal cell carcinoma (RCC) xenograft model since there is a lack of validated, non-invasive imaging tools for this purpose. 1 × 106 Caki-2 RCC cells were implanted under the renal capsule of 16 immunodeficient mice. Local and systemic tumor growth were monitored by regular hrUS, μCT and MRI examinations. Cells engrafted in all mice and gave rise to exponentially growing, solid tumors. All imaging techniques allowed to detect orthotopic tumors and to precisely calculate their volumes. While tumors appeared homogenously radiolucent in μCT, hrUS and MRI allowed for a better visualization of intratumoral structures and surrounding soft tissue. Examination time was the shortest for hrUS, followed by μCT and MRI. Tumor volumes determined by hrUS, μCT and MRI showed a very good correlation with each other and with caliper measurements at autopsy. 10 animals developed pulmonary metastases being well detectable by μCT and MRI. In conclusion, each technique has specific strengths and weaknesses, so the one(s) best suitable for a specific experiment may be chosen individually.
Spleen injuries are among the most frequent trauma-related injuries. At present, they are classified according to the anatomy of the injury. The optimal treatment strategy, however, should keep into consideration the hemodynamic status, the anatomic derangement, and the associated injuries. The management of splenic trauma patients aims to restore the homeostasis and the normal physiopathology especially considering the modern tools for bleeding management. Thus, the management of splenic trauma should be ultimately multidisciplinary and based on the physiology of the patient, the anatomy of the injury, and the associated lesions. Lastly, as the management of adults and children must be different, children should always be treated in dedicated pediatric trauma centers. In fact, the vast majority of pediatric patients with blunt splenic trauma can be managed non-operatively. This paper presents the World Society of Emergency Surgery (WSES) classification of splenic trauma and the management guidelines.

Hematopoietic differentiation is driven by transcription factors, which orchestrate a finely tuned transcriptional network. At bipotential branching points lineage decisions are made, where key transcription factors initiate cell type-specific gene expression programs. These programs are stabilized by the epigenetic activity of recruited chromatin-modifying cofactors. An example is the association of the transcription factor RUNX1 with protein arginine methyltransferase 6 (PRMT6) at the megakaryocytic/erythroid bifurcation. However, little is known about the specific influence of PRMT6 on this important branching point. Here, we show that PRMT6 inhibits erythroid gene expression during megakaryopoiesis of primary human CD34+ progenitor cells. PRMT6 is recruited to erythroid genes, such as glycophorin A. Consequently, a repressive histone modification pattern with high H3R2me2a and low H3K4me3 is established. Importantly, inhibition of PRMT6 by shRNA or small molecule inhibitors leads to upregulation of erythroid genes and promotes erythropoiesis. Our data reveal that PRMT6 plays a role in the control of erythroid/megakaryocytic differentiation and open up the possibility that manipulation of PRMT6 activity could facilitate enhanced erythropoiesis for therapeutic use.
Oxidative stress plays a fundamental role in many conditions. Specifically, redox imbalance inhibits endothelial cell (EC) growth, inducing cell death and senescence. We used global transcriptome profiling to investigate the involvement of noncoding-RNAs in these phenotypes. By RNA-sequencing, transcriptome changes were analyzed in human ECs exposed to H2O2, highlighting a pivotal role of p53-signaling. Bioinformatic analysis and validation in p53-silenced ECs, identified several p53-targets among both mRNAs and long noncoding-RNAs (lncRNAs), including MALAT1 and NEAT1. Among microRNAs (miRNAs), miR-192-5p was the most induced by H2O2 treatment, in a p53-dependent manner. Down-modulated mRNA-targets of miR-192-5p were involved in cell cycle, DNA repair and stress response. Accordingly, miR-192-5p overexpression significantly decreased EC proliferation, inducing cell death. A central role of the p53-pathway was also confirmed by the analysis of differential exon usage: Upon H2O2 treatment, the expression of p53-dependent 5’-isoforms of MDM2 and PVT1 increased selectively. The transcriptomic alterations identified in H2O2-treated ECs were also observed in other physiological and pathological conditions where redox control plays a fundamental role, such as ECs undergoing replicative senescence, skeletal muscles of critical limb-ischemia patients and the peripheral-blood mononuclear cells of long-living individuals. Collectively, these findings indicate a prominent role of noncoding-RNAs in oxidative stress response.
Hematopoietic differentiation is controlled by key transcription factors, which regulate stem cell functions and differentiation. TAL1 is a central transcription factor for hematopoietic stem cell development in the embryo and for gene regulation during erythroid/megakaryocytic differentiation. Knowledge of the target genes controlled by a given transcription factor is important to understand its contribution to normal development and disease. To uncover direct target genes of TAL1 we used high affinity streptavidin/biotin-based chromatin precipitation (Strep-CP) followed by Strep-CP on ChIP analysis using ChIP promoter arrays. We identified 451 TAL1 target genes in K562 cells. Furthermore, we analysed the regulation of one of these genes, the catalytic subunit beta of protein kinase A (PRKACB), during megakaryopoiesis of K562 and primary human CD34+ stem cell/progenitor cells. We found that TAL1 together with hematopoietic transcription factors RUNX1 and GATA1 binds to the promoter of the isoform 3 of PRKACB (Cβ3). During megakaryocytic differentiation a coactivator complex on the Cβ3 promoter, which includes WDR5 and p300, is replaced with a corepressor complex. In this manner, activating chromatin modifications are removed and expression of the PRKACB-Cβ3 isoform during megakaryocytic differentiation is reduced. Our data uncover a role of the TAL1 complex in controlling differential isoform expression of PRKACB. These results reveal a novel function of TAL1, RUNX1 and GATA1 in the transcriptional control of protein kinase A activity, with implications for cellular signalling control during differentiation and disease.
Sprifermin (rhFGF18) enables proliferation of chondrocytes producing a hyaline cartilage matrix

Objective: Fibroblast growth factor (FGF) 18 has been shown to increase cartilage volume when injected intra-articularly in animal models of osteoarthritis (OA) and in patients with knee OA (during clinical development of the recombinant human FGF18, sprifermin). However, the exact nature of this effect is still unknown. In this study, we aimed to investigate the effects of sprifermin at the cellular level.
Design: A combination of different chondrocyte culture systems was used and the effects of sprifermin on proliferation, the phenotype and matrix production were evaluated. The involvement of MAPKs in sprifermin signalling was also studied.
Results: In monolayer, we observed that sprifermin promoted a round cell morphology and stimulated both cellular proliferation and Sox9 expression while strongly decreasing type I collagen expression. In 3D culture, sprifermin increased the number of matrix-producing chondrocytes, improved the type II:I collagen ratio and enabled human OA chondrocytes to produce a hyaline extracellular matrix (ECM). Furthermore, we found that sprifermin displayed a ‘hit and run’ mode of action, with intermittent exposure required for the compound to fully exert its anabolic effect. Finally, sprifermin appeared to signal through activation of ERK.
Conclusions: Our results indicate that intermittent exposure to sprifermin leads to expansion of hyaline cartilage-producing chondrocytes. These in vitro findings are consistent with the increased cartilage volume observed in the knees of OA patients after intra-articular injection with sprifermin in clinical studies.
In lebende Körper zu sehen, ohne das Messer anzusetzen, das war lange ein Traum von Wissenschaftlern und Ärzten. Was vor mehr als 120 Jahren mit Conrad Röntgens Entdeckung der X-Strahlen begann, hat sich mit Magnetenzephalographie und Magnetresonanztomographie zu gängigen Instrumenten der Hightech-Medizin entwickelt.
Der negative Einfluss auf die Abstoßungsrate, die Transplantatfunktion und das Transplantatüberleben bei Nierentransplantationen von Antikörpern, die im Serum von Transplantatempfängern nachgewiesen werden, ist unumstritten. Die Entwicklung immer sensitiverer Methoden zur Antikörperdetektion wie beispielsweise die Luminex-Technologie wirft jedoch neue Fragen auf: Sind die hier nachgewiesenen, zum Teil sehr niedrigtitrigen Antikörper immer noch relevant für das Outcome von Transplantationen? Oder bergen die hier zusätzlich aufgedeckten Antikörper nur die Gefahr in sich, die Organallokation zu erschweren und mögliche komplikationslose Transplantationen zu verhindern? Zur Beantwortung dieser Fragen erfolgte die Durchführung dieser retrospektiven Studie.
In die Studie wurden Seren von 197 Nierentransplantatempfängern eingeschlossen. Diese Seren wurden vor der Transplantation auf Antikörper im Luminex untersucht und anschließend deren Transplantatfunktion und das Transplantatüberleben in Abhängigkeit vom Antikörperstatus ermittelt. Zusätzlich erfolgte eine Gegenüberstellung mit den Ergebnissen aus den im Rahmen der aktuellen Richtlinien zur Transplantationskompatibilität durchgeführten Testmethoden LCT und ELISA.
Bei einem Vergleich der drei unterschiedlichen Techniken zum HLA-Antikörpernachweis zeigte sich, dass der Luminex eine deutlich größere Sensitivität als der ELISA und insbesondere als der LCT besitzt. Außerdem war nachweisbar, dass die mittels Luminex Technology nachgewiesenen HLA-Antikörper ein schlechteres Transplantatüberleben bewirken (Graphik 4). Dies zeigte sich insbesondere, wenn HLA-Klasse I oder gleichzeitig HLA-Klasse I und HLA-Klasse II Antikörper bei Patienten vorhanden sind (Graphik 5). Im Gegensatz dazu fanden sich bei der Betrachtung der Patienten mit im LCT oder im ELISA nachgewiesen HLA-Antikörpern deutlich geringere, klinisch prognostische Hinweise auf das Transplantatüberleben (Graphik 10 und 13).
Das Transplantatüberleben war signifikant besser bei nicht-immunisierten Patienten als bei der Kohorte, die mittels einer der Nachweismethoden HLA-spezifische Antikörper aufwies. Eine Analyse der Transplantatfunktion, die mithilfe der Proteinurie und des Serum-Kreatinins bestimmt wurde, ergab, dass nicht immunisierte Patienten ein besser funktionierendes Transplantat besitzen als antikörperpositive Empfänger. Ein besonders negativer Einfluss donorspezifischer Antikörper konnte hier jedoch nicht nachgewiesen werden.
Im Hinblick auf die klinische Relevanz der nachgewiesenen HLA-Antikörper erfolgte die Auswertung mit unterschiedlichen MFI Kategorien (MFI 1000, 2000, 3000 und 4000). Anhand dieser Arbeit wurde ein MFI-Grenzwert zur Bestimmung des Antikörperstatus im Luminex von größer oder gleich 1000 im Luminex festgelegt, der am besten mit dem Transplantatoutcome korrelierte. Ein negativer Einfluss donorspezifischer Antikörper konnte jedoch nicht nachgewiesen werden. Es zeigte sich dennoch bei MFI Werten über 2000 ein tendenziell schlechteres Transplantatüberleben, wenn donorspezifische Antikörper vorliegen (Graphik 7).
Bei der Betrachtung des Antikörperstatus im LCT und im ELISA war kein Unterschied hinsichtlich des Transplantatüberlebens in Abhängigkeit vom Antikörperstatus sichtbar. Jedoch wurden die Patientenseren mit diesen beiden Methoden vor der Durchführung der Transplantation auf präformierte Antikörper untersucht und bei positiver Befundung eine intensivierte Immunsuppression veranlasst, was zu einer Beeinflussung des Transplantatoutcomes führt. Eine Unterteilung der nicht immunisierten Patienten im LCT und im ELISA anhand des Luminex-Ergebnisses zeigt, dass Patienten, die sowohl im LCT bzw. ELISA als auch im Luminex keine Antikörper aufweisen ein signifikant besseres Transplantatüberleben besitzen als Empfänger, die nur im Luminex immunisiert sind, jedoch nicht im LCT bzw. im ELISA. Dies lässt darauf schließen, dass die Beachtung von Antikörpern, die ausschließlich im Luminex nachgewiesen werden können, zu einem besseren Transplantatoutcome führt. Somit ist eine Durchführung des Luminex in Kombination mit anderen Antikörpernachweisverfahren von Vorteil.
Zur genauen Beantwortung der anfangs aufgeworfenen Fragen bedarf es sicherlich trotz einiger bereits vorhandener Arbeiten – mit zum Teil jedoch sehr unterschiedlichen Ergebnissen – noch zusätzlicher Studien. Auch sollte die Rolle der Denaturierung der HLA-Antigene auf den Beads, die zu falsch positiven und negativen Resultaten führen können, weiter analysiert werden. Außerdem muss der Einfluss komplementaktivierender Antikörper, wie mit der C1q- und C4d-Technik im Luminex nachweisbar, und der Einfluss nicht HLA-spezifischer Antikörper noch genauer erläutert werden.
BACKGROUND: Despite its impact on female health worldwide, no efforts have been made to depict the global architecture of ovarian cancer research and to understand the trends in the related literature. Hence, it was the objective of this study to assess the global scientific performance chronologically, geographically and in regards to economic benchmarks using bibliometric tools and density equalizing map projections.
METHODS: The NewQIS platform was employed to identify all ovarian cancer related articles published in the Web of Science since 1900. The items were analyzed regarding quantitative aspects (e.g. publication date, country of origin) and parameters describing the recognition of the work by the scientific community (e.g. citation rates).
RESULTS: 23,378 articles on ovarian cancer were analyzed. The USA had the highest activity of ovarian cancer research with a total of n = 9312 ovarian cancer-specific publications, followed by the UK (n = 1900), China (n = 1813), Germany (n = 1717) and Japan (n = 1673). Ovarian cancer-specific country h-index also showed a leading position of the USA with an h-index (HI) of 207, followed by the UK (HI = 122), Canada (HI = 99), Italy (HI = 97), Germany (HI = 84), and Japan (HI = 81). In the socio-economic analysis, the USA were ranked first with an average of 175.6 ovarian cancer-related publications per GDP per capita in 1000 US-$, followed by Italy with an index level of 46.85, the UK with 45.48, and Japan with 43.3. Overall, the USA and Western European nations, China and Japan constituted the scientific power players publishing the majority of highly cited ovarian cancer-related articles and dominated international collaborative efforts. African, Asian and South American countries played almost no visible role in the scientific community.
CONCLUSIONS: The quantity and scientific recognition of publications related to ovarian cancer are continuously increasing. The research endeavors in the field are concentrated in high-income countries with no involvement of lower-resource nations. Hence, worldwide collaborative efforts with the aim to exchange epidemiologic data, resources and knowledge have to be strengthened in the future to successfully alleviate the global burden related to ovarian cancer.
The sphingolipid sphingosine-1-phosphate (S1P) emerges as an important regulator of immunity, mainly by signaling through a family of five specific G protein-coupled receptors (S1PR1–5). While S1P signaling generally has the potential to affect not only trafficking but also differentiation, activation, and survival of a diverse range of immune cells, the specific outcome depends on the S1P receptor repertoire expressed on a given cell. Among the S1PRs, S1PR4 is specifically abundant in immune cells, suggesting a major role of the S1P/S1PR4 axis in immunity. Recent studies indeed highlight its role in activation of immune cells, differentiation, and, potentially, trafficking. In this review, we summarize the emerging data that support a major role of S1PR4 in modulating immunity in humans and mice and discuss therapeutic implications.
Autophagy is a membrane-trafficking process that directs degradation of cytoplasmic material in lysosomes. The process promotes cellular fidelity, and while the core machinery of autophagy is known, the mechanisms that promote and sustain autophagy are less well defined. Here we report that the epigenetic reader BRD4 and the methyltransferase G9a repress a TFEB/TFE3/MITF-independent transcriptional program that promotes autophagy and lysosome biogenesis. We show that BRD4 knockdown induces autophagy in vitro and in vivo in response to some, but not all, situations. In the case of starvation, a signaling cascade involving AMPK and histone deacetylase SIRT1 displaces chromatin-bound BRD4, instigating autophagy gene activation and cell survival. Importantly, this program is directed independently and also reciprocally to the growth-promoting properties of BRD4 and is potently repressed by BRD4-NUT, a driver of NUT midline carcinoma. These findings therefore identify a distinct and selective mechanism of autophagy regulation.
Invasive fungal infections are still an important cause of morbidity and mortality in immunocompromised patients such as patients suffering from hematological malignancies or patients undergoing hematopoietic stem cell transplantion. In addition, other populations such as human immunodeficiency virus-patients are at higher risk for invasive fungal infection. Despite the availability of new antifungal compounds and better supportive care measures, the fatality rate of invasive fungal infection remained unacceptably high. It is therefore of major interest to improve our understanding of the host–pathogen interaction to develop new therapeutic approaches such as adoptive immunotherapy. As experimental methodologies have improved and we now better understand the complex network of the immune system, the insight in the interaction of the host with the fungus has significantly increased. It has become clear that host resistance to fungal infections is not only associated with strong innate immunity but that adaptive immunity (e.g., T cells) also plays an important role. The antifungal activity of natural killer (NK) cells has been underestimated for a long time. In vitro studies demonstrated that NK cells from murine and human origin are able to attack fungi of different genera and species. NK cells exhibit not only a direct antifungal activity via cytotoxic molecules but also an indirect antifungal activity via cytokines. However, it has been show that fungi exert immunosuppressive effects on NK cells. Whereas clinical data are scarce, animal models have clearly demonstrated that NK cells play an important role in the host response against invasive fungal infections. In this review, we summarize clinical data as well as results from in vitro and animal studies on the impact of NK cells on fungal pathogens.
Background: Hereditary angioedema (HAE) is a rare genetic disease causing unpredictable and potentially life-threatening subcutaneous and submucosal edematous attacks. Cinryze® (Shire ViroPharma Inc., Lexington, MA, USA), a nanofiltered C1 inhibitor (C1-INH), is approved in Europe for the treatment, preprocedure prevention, and routine prophylaxis of HAE attacks, and for the routine prophylaxis of attacks in the USA. This phase 3 study assessed the safety and efficacy of 2 C1-INH doses in preventing attacks in children aged 6-11 years. Methods: A randomized single-blind crossover study was initiated in March 2014. Results for the first 6 patients completing the study are reported here. After a 12-week qualifying observation period, patients were randomly assigned to 1 of 2 C1-INH doses, 500 or 1,000 U, every 3-4 days for 12 weeks and crossed over to the alternative dose for a second 12-week period. The primary efficacy endpoint was the number of angioedema attacks per month. Results: Six females with HAE type I and a median age of 10.5 years received 2 doses of C1-INH (500 and 1,000 U). The mean (SD) difference in the number of monthly angioedema attacks between the baseline observation period and the treatment period was -1.89 (1.31) with 500 U and -1.89 (1.11) with 1,000 U. During the treatment periods, cumulative attack severity, cumulative daily severity, and the number of attacks needing acute treatment were lower. No serious adverse events or study drug discontinuations occurred. Conclusions: Interim findings from this study indicate that routine prevention with intravenous administration of C1-INH is efficacious, safe, and well tolerated in children ≥6 years of age.
Background
Cochlear Implants (CIs) provide near normal speech intelligibility in quiet environments to individuals suffering from sensorineural hearing loss. Perception of speech in situations with competing background noise and especially music appraisal however are still insufficient. Hence, improving speech perception in ambient noise and music intelligibility is a core challenge in CI research. Quantitatively assessing music intelligibility is a demanding task due to its inherently subjective nature. However, previous approaches have related electrophysiological measurements to speech intelligibility, a corresponding relation to music intelligibility, can be assumed. Recent studies have investigated the relation between results obtained from hearing performance tests and Spread of Excitations (SoEs) measurements. SoE functions are acquired by measuring Electrically Evoked Compound Action Potentials (ECAPs) which represent the electrical response generated in the neural structures of the auditory nerve. The parameters designed to describe SoE functions are used to estimate the dispersal of the electric field in the cochlea. The quality of spatial separation of the electrical field generated by adjacent electrodes are assumed to correlate with hearing performance measures.
Aim of study
This study investigated the relation of parameters derived by ECAP measurements and perceptive skills which aim to access the level of speech and music intelligibility in CI users. In addition, the ratings assessed in a questionnaire on self-rated music intelligibility were correlated to a test battery consisting of measures for speech reception threshold (SRT) in noise (Oldenburger Satztest (OLSA)) and music intelligibility (Adaptive Melody-Pattern-Discrimination Test (AMPDT)). We hypothesised that results from this test battery correlated to subjective ratings and measures describing SoE functions.
Methods
The patient collective covered 17 well-experienced bilateral CI listeners (8 females, 9 males) between the age of 14 and 77 years with a minimum CI experience of two years. Music enjoyment and self-rated musicality was evaluated by means of a questionnaire. The AMPDT included two psychoacoustic tests: timbre difference discrimination threshold (TDDT) and background contour discrimination threshold (BCDT). The accentuation of harmonics in a foreground melody created a background melody. Accentuation was realised by sound level increment, frequency detuning and onset asynchrony. Subjects had to detect target intervals comprising both foreground and background melody by discriminating timbre differences in a Three-Interval Three-Alternative Forced-Choice (3I3AFC) procedure. In a One-Interval Two-Alternative Forced-Choice (1I2AFC) procedure, subjects had to classify the background melody’s contour. SoE was measured via a spatial forward-masking paradigm. A basal, medial and apical recording electrode was measured. Probe electrodes were one electrode position apical to the recording. The width of normalised SoE functions was calculated at their 25 % and 50 % level (excitation distance (DIST)). Furthermore, exponential functions were calculated for SoE profiles with more than three data points for each side. The OLSA assessed SRT in noise. The noisy environment was presented through an array of four loudspeakers (MSNF). The Fastl noise-condition allows to make use of gap listening representing the temporal characteristics of speech as a fluctuating noise. The OLnoise-condition is a continuous noise resulting in a maximum portion of masking.
Results
We found that background melody contour classiffication (BCDT) is more challenging to CI users than the detection of small perceptual timbre differences (TDDT). Background melody contour classification was possible with harmonic accentuation by sound level increment whereas accentuation by onset asynchrony was more demanding. CI users failed in background melody contour classification obtained by frequency detuning. SRTs assessed in the OLSA were significantly lower in the OLnoise than in the Fastl noise masking condition. A number of N = 90 SoE functions were acquired from ECAP measurements, in which N = 48 showed a clearly present ECAP response. The DIST at the 25 % and 50 % level was narrower for the basal than for the apical and medial electrode. SoE functions showed asymmetric profiles with larger amplitudes towards the basal end of the cochlea. Correlation analysis between the AMPDT, OLSA and DISTs showed no significant correlation. Correlation analysis between the AMPDT, OLSA and the questionnaire’s results could not prove that musical activities (music listening, singing or playing instruments) improve music intelligibility. However, CI supply has restored the importance of music, self-rated musicality and musical enjoyment in this study’s subjects.
Conclusions
The present study’s results imply that CI listeners are only able to detect distinct timbre alterations throughout the course of a musical piece whereas they cannot discriminate background melodies hidden in a pattern of complex harmonic sounds. Furthermore, SoE measurements do not seem to be an adequate tool to predict neither speech nor music intelligibility in CI listeners, contrary to our initial hypothesis. This finding is consistent with a number of studies who did not find a correlation between music or speech intelligibility and channel interactions assessed by SoE measurements. It can be concluded that albeit CI supply restores musical enjoyment in patients with sensorineural hearing loss, music perception is still poor and does not significantly improve by regular musical activities such as listening to music, singing or playing instruments.
The etiology and pathogenesis of rheumatoid arthritis (RA) are marked by a complex interplay of various cell populations and is mediated by different signaling pathways. Traditionally, therapies have primarily focused on pain relief, reducing inflammation and the recovery of joint function. More recently, however, researchers have discussed the therapeutic efficacy of autologous platelet-rich plasma (PRP). The main objective of this work is to examine the influences of platelet-released growth factor (PRGF) on human synoviocytes under inflammatory conditions. Additionally, it is checked to which extend treatment with platelet concentrate influences the release of cytokines form synoviocytes. For this purpose, an in vitro RA model was created by stimulating the cells with the TNF-α. The release of cytokines was measured by ELISA. The cytokine gene expression was analyzed by real-time PCR. It has been observed that the stimulation concentration of 10 ng/ml TNF-α resulted in a significantly increased endogenous secretion and gene expression of IL-6 and TNF-α. The anti-inflammatory effect of PRGF could be confirmed through significant reduction of TNF-α and IL-1β. An induced inflammatory condition seems to cause PRGF to inhibit the release of proinflammatory cytokines. Further study is required to understand the exact effect mechanism of PRGF on synoviocytes.
Peripheral blood leukocytosis has been implicated in promoting tumor progression leading to worse survival, but the mechanisms behind this phenomenon remain unexplored. Here, we examined the prognostic role of pretreatment white blood cell (WBC) count and clinicopathologic parameters in the context of CD8+ tumor-infiltrating lymphocytes (TIL) and myeloperoxidase+ tumor-associated neutrophils (TANs) in patients with anal squamous cell carcinoma (ASCC) treated with definitive chemoradiotherapy (CRT). After a median follow-up of 26 months, leukocytosis correlated with advanced T-stage (p < 0.001) and N-stage (p < 0.001), and predicted for worse distant-metastasis-free survival (p = 0.006), disease-free-survival (DFS, p = 0.029), and overall survival (p = 0.013). Importantly, leukocytosis was associated with a lower intraepithelial CD8+ TIL density (p = 0.014), whereas low CD8+ TIL expression in the intraepithelial compartment was associated with worse DFS (p = 0.028). Additionally, high TAN expression in the peritumoral compartment was associated with a significantly lower density of CD8+ TIL (p = 0.039), albeit, TAN expression lacked prognostic value. In conclusion, leukocytosis constitutes an important prognostic marker in ASCC patients treated with CRT. In conjunction with intratumoral TIL and TAN, these data provide for the first time important insight on the correlation of peripheral blood leukocytosis with the intratumoral immune contexture and could be relevant for future patient stratification using immunotherapies in ASCC.
The role of endogenous melatonin for the control of the circadian system under entrained conditions and for the determination of the chronotype is still poorly understood. Mice with deletions in the melatoninergic system (melatonin deficiency or the lack of melatonin receptors, respectively) do not display any obvious defects in either their spontaneous (circadian) or entrained (diurnal) rhythmic behavior. However, there are effects that can be detected by analyzing the periodicity of the locomotor behaviors in some detail. We found that melatonin-deficient mice (C57Bl), as well as melatonin-proficient C3H mice that lack the melatonin receptors (MT) 1 and 2 (C3H MT1,2 KO), reproduce their diurnal locomotor rhythms with significantly less accuracy than mice with an intact melatoninergic system. However, their respective chronotypes remained unaltered. These results show that one function of the endogenous melatoninergic system might be to stabilize internal rhythms under conditions of a steady entrainment, while it has no effects on the chronotype.
Background: Cytogenetic aberrations such as deletion of chromosome 5q (del(5q)) represent key elements in routine clinical diagnostics of haematological malignancies. Currently established methods such as metaphase cytogenetics, FISH or array-based approaches have limitations due to their dependency on viable cells, high costs or semi-quantitative nature. Importantly, they cannot be used on low abundance DNA. We therefore aimed to establish a robust and quantitative technique that overcomes these shortcomings.
Methods: For precise determination of del(5q) cell fractions, we developed an inexpensive multiplex-PCR assay requiring only nanograms of DNA that simultaneously measures allelic imbalances of 12 independent short tandem repeat markers.
Results: Application of this method to n=1142 samples from n=260 individuals revealed strong intermarker concordance (R²=0.77–0.97) and reproducibility (mean SD: 1.7%). Notably, the assay showed accurate quantification via standard curve assessment (R²>0.99) and high concordance with paired FISH measurements (R²=0.92) even with subnanogram amounts of DNA. Moreover, cytogenetic response was reliably confirmed in del(5q) patients with myelodysplastic syndromes treated with lenalidomide. While the assay demonstrated good diagnostic accuracy in receiver operating characteristic analysis (area under the curve: 0.97), we further observed robust correlation between bone marrow and peripheral blood samples (R²=0.79), suggesting its potential suitability for less-invasive clonal monitoring.
Conclusions: In conclusion, we present an adaptable tool for quantification of chromosomal aberrations, particularly in problematic samples, which should be easily applicable to further tumour entities.
Einfluss der natürlichen Verbindung Curcumin auf die Invasion von Harnblasenkarzinomzellen in vitro

Das Harnblasenkarzinom macht 3,4 % aller bösartigen Tumore aus. In Deutschland erkranken jedes Jahr 16.000 Menschen mit stetig steigenden Inzidenzraten. Betroffen sind vor allem äl-tere Menschen.
Die Therapie erfolgt in Abhängigkeit vom Tumorstadium. Im fortgeschrittenen Stadium wird der Patient i. d. R. mit zwei bis drei Zyklen MVAC (Methotrexat, Vinblastin, Doxrubicin und Cisplatin) behandelt. Unter Chemotherapie lässt sich die Überlebenszeit des Patienten verlän-gern, dennoch schränken rasch aufkommende Resistenzen den Wirkungsgrad erheblich ein. Curcumin kann den Bedarf von Patienten, die Krankheit zusätzlich aktiv bekämpfen zu können, decken. Die natürliche Substanz hat das Potenzial, sich als eine zusätzliche Therapieoption beim fortgeschrittenen Harnblasenkarzinom zu etablieren. Ziel der Studie war es, die Wirk-samkeit von Curcumin durch Lichtexposition zu erhöhen und die Bedeutung dieses Ansatzes für die Hemmung der Tumorinvasion anhand von Harnblasenkarzinomzellen zu evaluieren.
Zellkulturen der Harnblasenkarzinomzelllinien UMUC3, RT112 und TCCSUP wurden angelegt. Die Tumorzellen wurden vor den Versuchen mit Curcumin in verschiedenen Konzentrationen (0,05 μg/ml, 0,1 μg/ml und 0,2 μg/ml) und/oder mit Tageslicht behandelt und in entspre-chende Proben eingeteilt. Mittels Adhäsionsversuchen wurde das Bindungsverhalten der Krebszellen an extrazelluläre Matrixproteine (Kollagen, Fibronektin) und humane Endothelzel-len (HUVEC) untersucht. Die Migrationstendenz wurde mithilfe eines Chemotaxisexperiments ermittelt. Um die Wirkung von Curcumin auf die für den Metastasierungsprozess relevanten α- und β-Integrin-Subtypen zu untersuchen, wurde eine FACScan-Analyse durchgeführt. Der Western Blot diente zur Quantifizierung der Integrinproteine vor und nach Behandlung. Zu-sätzlich wurde durch Blockadeanalysen dargestellt, in welchem Rahmen die Tumorzellen in ihrem Bindungs- und Migrationsverhalten von selektiven Integrinen abhängig sind.
Eine Bestrahlung von Curcumin mit Tageslicht (CurcuminLicht) hatte eine signifikante Wirkungs-steigerung der Substanz gegenüber unbestrahltem Curcumin zur Folge. Einheitlich verringerte CurcuminLicht die Adhäsion sämtlicher Tumorzelllinien an immobilisiertes Kollagen und Fibronektin. Die Interaktion von UMUC3 und RT112 mit HUVEC wurde gehemmt, hingegen er-höhte sich die Bindungsrate von TCCSUP signifikant gegenüber der unbehandelten Kontrolle. Die Tumorzellmotilität (Chemotaxis) wurde in Gegenwart von CurcuminLicht deutlich geblockt. Die FACScan-Analyse und Western Blotting verwiesen auf distinkte Modulationen der α- und β-Integrin-Subtypen. Diese waren in Abhängigkeit vom Tumorzelltyp unterschiedlich ausge-prägt. Mittels Blockadestudien konnte demonstriert werden, dass die Integrin-Subtypen α3, α5 und β1 wesentlich in Adhäsions- und Migrationsereignisse eingebunden sind.
Die Ergebnisse verweisen auf die Bedeutung der Lichtexposition für die Wirkeffektivität der Natursubstanz Curcumin. CurcuminLicht vermag, invasive Prozesse im Rahmen der Tumordis-semination zu unterbinden, und könnte sich somit als wertvolles Additivum im Rahmen der konventionellen Tumorbehandlung erweisen. In nachfolgenden Studien sollten die erarbeite-ten Daten am Tiermodell verifiziert werden.
Etablierung innovativer Strategien zur Resistenzumgehung bei der Therapie des Urothelkarzinoms

Patienten mit fortgeschrittenem Blasenkarzinom haben eine sehr schlechte Prognose. Neue Behandlungsansätze, die ihnen eine realistische Heilungschance bieten, sind dringend notwendig. Der Einsatz des mTOR-Inhibitors TEM könnte ein innovatives Lösungskonzept darstellen. Zu berücksichtigen ist jedoch die Gefahr einer Resistenzinduktion unter Langzeitbehandlung. Basierend auf vorausgegangenen Studien wurde im Rahmen der Promotionsarbeit postuliert, dass die Behandlung mit dem HDAC-Inhibitor VPA eine Therapiestrategie darzustellen vermag, mit der durch Tem bedingte Resistenzen umgegangen werden können.
In der vorliegenden Arbeit wurde das biologische Verhalten verschiedener Blasenkarzinomzellen unter chronischer Tem-Therapie studiert und anschließend der Einfluss von VPA auf Wachstum und Proliferation der Zellen (TEM-resistent versus TEM-sensibel) untersucht.
Verwendet wurden die Zelllinien RTT112 und UMUC3. Die Resistenz wurde durch TEM induziert. Das Zellwachstum wurde mit MTT und die Proliferation mittels BrdU-Einbau evaluiert. Mittels Flow-Zytometrie erfolgte die Analyse der Zellzyklusprogression. Relevante Zellzyklusproteine und intrazelluläre Zielproteine wurden mittels Western Blot Verfahren analysiert. siRNA Knock-Down-Studien dienten zur Beurteilung der biologischen Relevanz der Proteine.
Die chronische mTOR-Blockade mit TEM über mehrere Monate hinweg ging mit einer Resistenzinduktion einher, nachgewiesen über eine Verschiebung des IC50-Wertes. VPA bewirkte nicht nur bei den TEM-sensiblen, sondern auch den TEM-resistenten Zellen eine signifikante Wachstums- und Proliferationshemmung. Der Effekt korrelierte mit einer Zunahme von Tumorzellen in der G0/G1-Phase des Zellzyklusses und Abnahme von S-Phase-Zellen. Die G2/M-Phase reduzierte sich in RT112-Kulturen und bei TEM-sensitiven UMUC3-Zellen. Hingegen erhöhte sich der Anteil von TEM-resistenten UMUC3-Kulturen in G2/M. VPA bewirkte bei allen Zellpopulationen Veränderungen an der Cyclin-Cdk-Achse. Insbesondere bei UMUC3-Zellen kam es unter VPA zur Suppression in der Aktivität mTOR-relevanter Signalmoleküle (Rictor, Raptor, Akt, p70S6). In beiden Zelllinien, RT112 und UMUC3, erhöhte sich der Expressionsspiegel der acetylierten Histone H3 und H4. Mittels siRNA wurde belegt, dass die Blockade von Cyclin A und Cdk2 mit einem verminderten Tumorwachstum assoziiert ist.
Aufgrund der vorgestellten Daten lässt sich postulieren, dass die Anwendung von VPA die Gefahr einer durch TEM induzierten Resistenz abzuwenden vermag. Die Ergebnisse der vorliegenden Arbeit präsentieren vielversprechende anti-tumorale Effekte VPAs und stellen möglicherweise eine innovative und effiziente Strategie zur Behandlung des fortgeschrittenen Blasenkarzinoms dar. Da Tem und VPA in der Klinik bereits etabliert sind, wäre eine Umsetzung rasch möglich.
Zur Verifizierung der am in vitro Modell erhobenen Daten sind weitere, vertiefende Untersuchungen am lebenden Organismus essentiell, um die Resistenzphänomene zu belegen.
Recent studies have suggested increased plasticity of differentiated cells within the intestine to act both as intestinal stem cells (ISCs) and tumour-initiating cells. However, little is known of the processes that regulate this plasticity. Our previous work has shown that activating mutations of Kras or the NF-κB pathway can drive dedifferentiation of intestinal cells lacking Apc. To investigate this process further, we profiled both cells undergoing dedifferentiation in vitro and tumours generated from these cells in vivo by gene expression analysis. Remarkably, no clear differences were observed in the tumours; however, during dedifferentiation in vitro we found a marked upregulation of TGFβ signalling, a pathway commonly mutated in colorectal cancer (CRC). Genetic inactivation of TGFβ type 1 receptor (Tgfbr1/Alk5) enhanced the ability of KrasG12D/+ mutation to drive dedifferentiation and markedly accelerated tumourigenesis. Mechanistically this is associated with a marked activation of MAPK signalling. Tumourigenesis from differentiated compartments is potently inhibited by MEK inhibition. Taken together, we show that tumours arising in differentiated compartments will be exposed to different suppressive signals, for example, TGFβ and blockade of these makes tumourigenesis more efficient from this compartment.
Background: All European countries need to increase the number of health professionals in the near future. Most efforts have not brought the expected results so far. The current notion is that this is mainly related to the fact that female physicians will clearly outnumber their male colleagues within a few years in nearly all European countries. Still, women are underrepresented in leadership and research positions throughout Europe.
Objectives: The MedGoFem project addresses multiple perspectives with the participation of multiple stakeholders. The goal is to facilitate the implementation of Gender Equality Plans (GEP) in university hospitals; thereby, transforming the working conditions for women working as researchers and highly qualified physicians simultaneously. Our proposed innovation, a crosscutting topic in all research and clinical activities, must become an essential part of university hospital strategic concepts.
Methods: We capture the current status with gender-sensitive demographic data concerning medical staff and conduct Web-based surveys to identify cultural, country-specific, and interdisciplinary factors conducive to women’s academic success. Individual expectations of employees regarding job satisfaction and working conditions will be visualized based on “personal construct theory” through repertory grids. An expert board working out scenarios and a gender topic agenda will identify culture-, nation-, and discipline-specific aspects of gender equality. University hospitals in 7 countries will establish consensus groups, which work on related topics. Hospital management supports the consensus groups, valuates group results, and shares discussion results and suggested measures across groups. Central findings of the consensus groups will be prepared as exemplary case studies for academic teaching on research and work organization, leadership, and management.
Results: A discussion group on gender equality in academic medicine will be established on an internationally renowned open-research platform. Project results will be published in peer-reviewed journals with high-impact factors. In addition, workshops on gender dimension in research using the principles of Gendered Innovation will be held. Support and consulting services for hospitals will be introduced in order to develop a European consulting service.
Conclusions: The main impact of the project will be the implementation of innovative GEP tailored to the needs of university hospitals, which will lead to measurable institutional change in gender equality. This will impact the research at university hospitals in general, and will improve career prospects of female researchers in particular. Simultaneously, the gender dimension in medical research as an innovation factor and mandatory topic will be strengthened and integrated in each individual university hospital research activity. Research funding organizations can use the built knowledge to include mandatory topics for funding applications to enforce the use and implementation of GEP in university hospitals.
T lymphocytes against tumor-specific mutated neoantigens can induce tumor regression. Also, the size of the immunogenic cancer mutanome is supposed to correlate with the clinical efficacy of checkpoint inhibition. Herein, we studied the susceptibility of tumor cell lines from lymph node metastases occurring in a melanoma patient over several years towards blood-derived, neoantigen-specific CD8+ T cells. In contrast to a cell line established during early stage III disease, all cell lines generated at later time points from stage IV metastases exhibited partial or complete loss of HLA class I expression. Whole exome and transcriptome sequencing of the four tumor lines and a germline control were applied to identify expressed somatic single nucleotide substitutions (SNS), insertions and deletions (indels). Candidate peptides encoded by these variants and predicted to bind to the patient’s HLA class I alleles were synthesized and tested for recognition by autologous mixed lymphocyte-tumor cell cultures (MLTCs). Peptides from four mutated proteins, HERPUD1G161S, INSIG1S238F, MMS22LS437F and PRDM10S1050F, were recognized by MLTC responders and MLTC-derived T cell clones restricted by HLA-A*24:02 or HLA-B*15:01. Intracellular peptide processing was verified with transfectants. All four neoantigens could only be targeted on the cell line generated during early stage III disease. HLA loss variants of any kind were uniformly resistant. These findings corroborate that, although neoantigens represent attractive therapeutic targets, they also contribute to the process of cancer immunoediting as a serious limitation to specific T cell immunotherapy.
The Gini index is a measure of the inequality of a distribution that can be derived from Lorenz curves. While commonly used in, e.g., economic research, it suffers from ambiguity via lack of Lorenz dominance preservation. Here, investigation of large sets of empirical distributions of incomes of the World’s countries over several years indicated firstly, that the Gini indices are centered on a value of 33.33% corresponding to the Gini index of the uniform distribution and secondly, that the Lorenz curves of these distributions are consistent with Lorenz curves of log-normal distributions. This can be employed to provide a Lorenz dominance preserving equivalent of the Gini index. Therefore, a modified measure based on log-normal approximation and standardization of Lorenz curves is proposed. The so-called UGini index provides a meaningful and intuitive standardization on the uniform distribution as this characterizes societies that provide equal chances. The novel UGini index preserves Lorenz dominance. Analysis of the probability density distributions of the UGini index of the World’s counties income data indicated multimodality in two independent data sets. Applying Bayesian statistics provided a data-based classification of the World’s countries’ income distributions. The UGini index can be re-transferred into the classical index to preserve comparability with previous research.
Am Fachbereich Medizin und dem Klinikum der Johann Wolfgang-Goethe-Universität Frankfurt existierten bereits seit 2002 mehrere einzelne medizindidaktische Kurse. Diese Aktivitäten wurden 2011 strukturiert, ein umfassendes Kursangebot, das das breite Spektrum an Themen rund um die Lehre abdeckt, wurde aufgebaut und unter dem Dach der Frankfurter Arbeitsstelle für Medizindidaktik (FAM) am Fachbereich institutionalisiert. Folgende Faktoren waren für die erfolgreiche Umsetzung ausschlaggebend: vorhandene Programme in anderen Bundesländern (v.a. Baden-Württemberg, Nordrhein-Westfalen) mit entsprechenden Vorgaben, die Unterstützung der Studiendekane, die Verankerung der Teilnahme an medizindidaktischen Kursen in der Habilitationsordnung sowie eine kritische Masse von an der Lehre interessierten Mitarbeiterinnen und Mitarbeitern. Kernelemente des Angebots sind ein Basiskurs für alle neu eingestellten wissenschaftlichen Angestellten mit Lehrverpflichtung und ein modularer Aufbau des Programms, der individuellen Präferenzen bzw. Erfordernissen entgegen kommt. Gleichwohl die Teilnahme am Kursprogramm überwiegend verpflichtend erfolgt, zeigt sich eine hohe Zufriedenheit und ein nachhaltiger Wissenszuwachs bei den Kursteilnehmerinnen und Kursteilnehmern.
Tumor cells frequently overexpress heat shock protein 70 (Hsp70) and present it on their cell surface, where it can be recognized by pre‐activated NK cells. In our retrospective study the expression of Hsp70 was determined in relation to tumor‐infiltrating CD56+ NK cells in formalin‐fixed paraffin embedded (FFPE) tumor specimens of patients with SCCHN (N = 145) as potential indicators for survival and disease recurrence. All patients received radical surgery and postoperative cisplatin‐based radiochemotherapy (RCT). In general, Hsp70 expression was stronger, but with variable intensities, in tumor compared to normal tissues. Patients with high Hsp70 expressing tumors (scores 3–4) showed significantly decreased overall survival (OS; p = 0.008), local progression‐free survival (LPFS; p = 0.034) and distant metastases‐free survival (DMFS; p = 0.044), compared to those with low Hsp70 expression (scores 0–2), which remained significant after adjustment for relevant prognostic variables. The adverse prognostic value of a high Hsp70 expression for OS was also observed in patient cohorts with p16‐ (p = 0.001), p53‐ (p = 0.0003) and HPV16 DNA‐negative (p = 0.001) tumors. The absence or low numbers of tumor‐infiltrating CD56+ NK cells also correlated with significantly decreased OS (p = 0.0001), LPFS (p = 0.0009) and DMFS (p = 0.0001). A high Hsp70 expression and low numbers of tumor‐infiltrating NK cells have the highest negative predictive value (p = 0.00004). In summary, a strong Hsp70 expression and low numbers of tumor‐infiltrating NK cells correlate with unfavorable outcome following surgery and RCT in patients with SCCHN, and thus serve as negative prognostic markers.
In der vorliegenden Studie wurden sieben verschiedene Biomarker (Survivin, Ki-S2, EGFR, Her2/neu, PTEN, p53, OSF-2) von Plattenepithelkarzinomen der Mundhöhle und des anterioren Oropharynx bezüglich ihres Expressionsverhaltens vor und nach Applikation einer intraarteriellen Induktionschemotherapie mit 150mg/m² Cisplatin und im Hinblick auf einen möglichen Zusammenhang des Expressionsverhaltens mit dem Chemotherapieresponse untersucht.
Mit Hilfe der TMA-Technologie wurde in Gewebeproben von 41 Patienten, welche am genannten Krebs erkrankt waren, die Expressionsraten immunhistochemisch bestimmt. Die Behandlung der Patienten erfolgte im Rahmen eines multimodalen Therapiekonzepts in der Klinik für Mund-, Kiefer- und Plastische Gesichtschirurgie am Klinikum der Johann Wolfgang Goethe-Universität in Frankfurt am Main. Alle Patienten hatten eine intraarterielle Induktionschemotherapie erhalten gefolgt von einer chirurgischen Intervention. Spätestens nach sechs Wochen erfolgte die adjuvante Bestrahlung beziehungsweise Radiochemotherapie (Maximalvariante). 56% der Patienten erhielten die Maximalvariante.
Die allgemeine pathologische Ansprechrate nach intraarterieller Induktion lag bei 39% (2%= pCR, 37%= pPR). 59% der Patienten zeigten kein Ansprechen auf die Induktion (pSD) und 2% zeigten eine Progression (pPD). Die Expressionsraten der einzelnen Biomarker wurden jeweils vor und nach intraarterieller Induktionschemotherapie bestimmt. Die statistische Analyse erfolgte univariat mittels Wilcoxon-Mann-Whitney-U-Test und Wilcoxon-matched-pairs-Test. Für Survivin, Ki-S2, OSF-2 und EGFR konnten signifikante Unterschiede nach Induktion verzeichnet werden, die Expressionsrate stieg für Survivin Kern (p= 0,0001), OSF-2 (OSF-2 Zytoplasma, p= 0,0067; OSF-2 Membran, p= 0,0001) und EGFR (p= 0,039) signifikant an. Für Ki-S2 nahm die Expressionsrate signifikant ab (p= 0,001).
In der Vergleichsgruppe Responder versus Non-Responder konnten sowohl in der diagnostischen Primärtumorbiopsie als auch im OP-Präparat des Primärtumors nach Induktion keine signifikanten Unterschiede zwischen Respondern und Non-Respondern in den Expressionsraten festgestellt werden. Für Survivin Zytoplasma lagen die Expressionsraten für die Responder-Patienten vor und nach Induktion im oberen Bereich. Für PTEN konnte bei 75% der Patienten ein Expressionsverlust verzeichnet werden. Damit konnte in der hier durchgeführten Studie kein Biomarker detektiert werden, welcher für eine signifikante Vorhersage zur Responserate herangezogen werden könnte.
Die Änderung der Expressionsraten von Biomarkern bei oralen und oropharyngealen Plattenepithelkarzinomen nach intraarterieller Induktionschemotherapie ist ein Befund, der an ähnliche Beobachtungen nach neoadjuvanter Chemotherapie z. B. beim Mammakarzinom anknüpft und darum weiter verfolgt werden sollte.
