Identification of translationally deregulated proteins during inflammation-associated tumorigenesis

The translation of mRNAs into proteins is an elaborate and highly regulated process. Translational regulation primarily takes place at the level of initiation. During initation the eukaryotic initiation factors (eIFs) form a complex that binds to the 5’end of the mRNA to scan for a start codon. Once recognized, the ribosome is recruited to the mRNA and protein synthesis starts. Initiation of translation can basically occur via two distinct mechanisms, i.e. cap-dependent and cap-independent that is mediated via internal ribosome entry sites (IRESs). The former is mediated by a 5’cap structure composed of a 7-methylguanylate which is added to every mRNA during transcription and recruits the initiation complex. IRES-dependent translation involves elements within the 5’untranslated region (UTR) of the mRNA that mostly bind IRES trans-acting factors (ITAFs) which associate either with the initiation complex or with the ribosome itself and consequently allow for internal initiation of translation.
During tumorigenesis the demand for proteins is increased due to rapid cell growth, which consequently requires enhanced translation. Many factors that regulate translation are overexpressed in tumors. Moreover, signaling pathways that trigger translation or further hyperactivated by the surrounding tumor microenvironment. This environment is largely generated by infiltration of immune cells such as macrophages that secrete cytokines and other mediators to promote tumorigenesis. As the effects of inflammatory conditions on the translation of specific targets are only poorly characterized, my study aimed at identifying translationally deregulated targets during inflammation-associated tumorigenesis.
For this purpose, I cocultured MCF7 breast tumor cells with conditioned medium of activated monocyte-derived U937 macrophages (CM). Polysome profiling and microarray analysis identified 42 targets to be regulated at the level of translation. The results were validated by quantitative PCR and one target - early growth response 2 (EGR2) - was chosen for in depth analysis of the mechanism leading to its enhanced translation.
In order to identify upstream signaling molecules causing enhanced EGR2 protein synthesis the cytokine profile of CM was analyzed and the impact of several cytokines on EGR2 translation was examined. Preincubation of CM with neutralizing antibodies revealed that lowering interleukin 6 (IL-6) had only little effect, whereas depletion of IL 1β significantly reduced EGR2 translation. This finding was corroborated by the fact that treatment with recombinant IL-1β enhanced EGR2 translation to virtually the same extend as CM. Further experiments revealed that this effect was mediated via the p38-MAPK signaling cascade.
Interestingly, I observed that the mTOR inhibitor rapamycin, which reduces cap-dependent translation, specifically stimulated EGR2 translation. This result argued for an IRES-dependent mechanism that might account for EGR2 translation. The use of bicistronic reporter assays verified this hypothesis. In line with the above mentioned results, CM, IL-1β and p38-MAPK induced EGR2-IRES activity.
Since IRESs commonly require ITAFs to mediate translation initiation, the binding of proteins to the 5’UTR was analyzed using mass spectrometry. Among others, several previously described ITAFs, such as polypyrimidine tract-binding protein (PTB) and heterogeneous nuclear ribonucleoprotein A1 (hnRNP-A1) were identified to directly bind to the EGR2-5’UTR. Furthermore, overexpression of hnRNP-A1 enhanced EGR2-IRES activity whereas a dominant negative form of hnRNP-A1 significantly decreased it, thus, showing its importance for EGR2 translation.
In summary, my data provide evidence that EGR2 expression can be controlled by IRES-dependent translational regulation, which is responsive to an inflammatory environment. The identified mechanism may not be exclusive for one target but might be representative for gene expression regulation mechanisms during tumorigenesis. This is of special interest for the treatment of cancer patients and development of more specific therapies to reduce tumor outcome.
Ein charakteristisches Merkmal von Autoimmunerkrankungen ist der selektive Angriff des Immunsystems auf einen einzigen Zelltyp, ein Organ oder Gewebe durch bestimmte T- und B-Lymphozyten. Die genaue Ursache von Autoimmunerkrankungen bleibt bislang ungeklärt, jedoch scheint eine bestimmte genetische Konstellation bei Zusammentreffen mit im weitesten Sinne umweltbedingten Faktoren die Reaktionen zu erklären. Zu solchen umweltbedingten Faktoren gehören auch Infektionen. Häufig werden Assoziationen zwischen Infektion und autoimmuner Erkrankung beobachtet. Auch kann bei einer akuten Infektion ein vorhandener Autoimmunprozess exazerbieren. Eine schnelle Verteidigung gegen Infektionen gewährleistet das angeborene Immunsystem, bevor das adaptive Immunsystem zum Tragen kommt. Die Moleküle, die bei diesen Abwehrprozessen frei werden, können zur T-Zell-Vermehrung führen, die einerseits eine effektive Selbstverteidigung möglich macht; ander erseits können sie aber auch die Vermehrung autoreaktiver T-Zellen zur Folge haben. An frühen Vorgängen nach dem Kontakt mit einem Pathogen ist eine Vielzahl von Zyto- und Chemokinen beteiligt. Typ-1-Interferone (IFN1) sind ebensolche Zytokine, die vor allem in der Abwehr gegen Viren eine Rolle spielen und in mehrschrittigen Prozessen auch zur Aktivierung von T- und B-Zellen führen. IFIH1 ist ein intrazellulärer Rezeptor, der dsRNA im Zytosol detektiert und in der Folge die Transkription von IFN1 aktiviert. Veränderungen im Gen dieses Rezeptors zeigen Assoziationen zu Typ-1-Diabetes (T1D) und bieten einen funktionellen Berührungspunkt zwischen früher Infektabwehr durch das angeborene Immunsystem und der letztlich gewebszerstörerischen T- und B-Zell-Reaktion. In dieser Arbeit wurde daher die Assoziation dreier endokriner Autoimmunopathien mit dem IFH1-Polymorphismus rs1990760 durchgeführt, der in der Literatur als deutlich assoziiert mit T1D beschrieben wird. Mit Hashimoto-Thyreoditis, Morbus Basedow und Morbus Addison wurden Krankheiten gewählt, die im genetischen Hintergrund viele Gemeinsamkeiten untereinander und mit T1D aufweisen. Durch die in der Literatur beschriebenen Assoziationen mit T1D rückt nun der IFIH1-rs1990760 Polymorphismus potentiell auch in das genetische Anfälligkeitsprofil dieser endokrinen Autoimmunopathien. Für die Assoziationsanalyse wurde in dieser Arbeit DNA von betroffenen Individuen und gesunden Kontrollen mittels RT-PCR auf den IFIH1-Polymorphismus rs1990760 (Allele „A“/„G“) typisiert und die Genotyp- und Allelhäufigkeiten verglichen. Außerdem wurden Familienanalysen durchgeführt und die Transmission der Allele auf Unterschiede im Vererbungsverhalten hin untersucht. Es konnten keine statistisch relevanten Assoziationen zwischen dem IFIH1-Polymorphismus rs1990760 und Hashimoto-Thyreoditis, Morbus Basedow und Morbus Addison gefunden werden. Weiterhin wurden Subgruppen gebildet, um eventuell in der Gesamtgruppe nicht erkennbare Trends aufzudecken. Für Hashimoto-Thyreoditis und Morbus Basedow wurde der HLA-Typ DQ2 als Risikotyp angenommen, für Morbus Addison der Typ HLA-DQ2/DQ8. Der Antikörperstatus der untersuchten Individuen wurde ebenfalls in die Untersuchung einbezogen. In der Familienanalyse mit an Hashimoto-Thyreoditis erkranktem Kind wurden Subgruppen gebildet, die den TPO-Ak und Tg-Ak-Status der Eltern berücksichtigen. Bei Morbus Basedow wurde der TSH-R-Antikörper bei der Fall-Kontroll-Analyse zur Subgruppenbildung herangezogen. Wenn sich auch keine statistisch eindeutigen Assoziationen nachweisen ließen, so waren bei Hashimoto-Thyreoditis und Morbus Basedow in einigen Subgruppen doch Tendenzen zu erkennen, die eine Beteiligung des Polymorphismus an der genetischen Anfälligkeit vermuten lassen könnten. Die beobachteten Trends deuten darauf hin, dass in bestimmten Patientengruppen der untersuchte Polymorphismus eine Rolle in der Anfälligkeit spielen könnte.
Ubiquitin-binding modules are constituents of cellular proteins that mediate the effects of ubiquitylation by making transient, non-covalent interactions with ubiquitin molecules. While some ubiquitin-binding modules bind single ubiquitin moieties, others are selective for specific ubiquitin chains of different linkage types and lengths. In recent years, functions of ubiquitin chains that are polymerized through their Lys or N-terminal Met (i.e. linear chains) residues have been linked to a variety of cellular processes. Selectivity of ubiquitin-binding modules for different ubiquitin chain types appears as a key to the distinct regulatory consequences during protein quality control pathways, receptor endocytosis, gene transcription, signaling via the NF-κB pathway, and autophagy.
Background: Radiofrequency ablation is a minimal invasive therapy in the treatment of bone metastases. In this study we present a new ablation system enabling an ablation in multiple directions and with an adaptable size and shape.
Material and methods: VX-2 tumor was used for the induction of experimental bone metastases in the femur of six New Zealand white rabbits. X-ray imaging as well as CT and MRI scans before and after treatment was carried out. After detecting bone tumor, radiofrequency ablation was performed. The ablation instrument contained a 10 g bipolar, articulated extendable electrode and a proprietary generator with an impedance controlled algorithm. All bones and the soft tissue were examined histologically.
Results: All animals developed local bone tumor. Mean duration until first osteolytic lesions on CT-scans was 48±14 days. The mean lesion area was 26 mm(2). No systemic tumor spread was seen. 6 radiofrequency procedures were carried out with a mean application time of 6 min±2:30 and an average temperature in the region of effect of 55 °C±4. MRI imaging demonstrated an ablation zone of 23±6 mm around the electrode. Histopathology showed an extensive heat necrosis with no remaining tumor cells in the ablation area.
Conclusion: Radiofrequency ablation is a quickly developing treatment option on the field of minimal invasive bone tumor therapy. The electrode enables an ablation adapted to size and shape of the metastases. Further clinical studies are necessary to test and enhance this radiofrequency system.
Introduction: We report on successful endovascular treatment of a hydrofluoric acid burn to the hand.
Report: A worker complained of severe pain in the fingers D II to D V after injury with 60% hydrofluoric acid. A digital subtraction angiography showed vasospasm of the common palmar digital artery. We selectively applied 20% calcium gluconate intra-arterially.
After treatment all arteries were perfused. Alprostadil, acetylsalicylic acid and clopidogrel were administered in conjunction. Pain symptoms improved and sensory and motor functions were restored.
Discussion: Immediate angiography and intra-arterial application of calcium gluconate are recommended to treat hydrofluoric acid burn to a limb.
Inhibitor of apoptosis (IAPs) proteins are characterized by the presence of evolutionarily conserved baculoviral inhibitor of apoptosis repeat (BIR) domains, predominantly known for their role in inhibiting caspases and, thereby, apoptosis. We have shown previously that multi-BIR domain-containing IAPs, cellular IAPs, and X-linked IAP can control tumor cell migration by directly regulating the protein stability of C-RAF kinase. Here, we extend our observations to a single BIR domain containing IAP family member melanoma-IAP (ML-IAP). We show that ML-IAP can directly bind to C-RAF and that ML-IAP depletion leads to an increase in C-RAF protein levels, MAPK activation, and cell migration in melanoma cells. Thus, our results unveil a thus far unknown role for ML-IAP in controlling C-RAF stability and cell migration.
Die Gegenübertragung hat sich mittlerweile zu einem der wichtigsten Instrumente der stationären psychosomatischen Therapie entwickelt. Ihr kommt in der psychodynamischen Psychotherapie für das Verständnis der unbewussten Konflikte und für den damit zusammenhängenden Behandlungserfolg eine zentrale Funktion zu. Dies gilt für die Einzeltherapie, aber auch für die integrative stationäre psychodynamische Therapie und deren „Herzstück“ (Janssen 2004) - das multiprofessionelle Team. Die Ziele der Arbeit bestehen - abgesehen von der Beschreibung der Faktorenstruktur und Reliabilität des Gegenübertragungsfragebogens - darin, herauszufinden, ob sich a) die Gegenübertragung in unterschiedlichen Therapieverfahren unterscheidet, b) in welcher Weise die Gegenübertragung mit dem Beziehungserleben und der Beziehungsgestaltung des Patienten zusammenhängt, c) ob die Gegenübertragung mit der Belastung des Patienten zusammenhängt und d) ob sich die Gegenübertragung abhängig von der Diagnose unterscheidet.
Methode: Dazu wurde mithilfe des Gegenübertragungsfragebogens (CTQ-D)
die Gegenübertragung von 137 Patienten durch mehrere Therapeuten (Ärzte, Psychologen und Pflegekräfte) aus zwei psychosomatischen Kliniken zu Therapiebeginn und zum Therapieende erhoben. Insgesamt flossen 1131 Fragebögen in die Auswertung mit ein. Die Faktorenanalyse des CTQ-D ergab eine Lösung mit sieben statistisch und klinisch kohärenten Faktoren: 1) aggressiv-resignative GÜ, 2) positiv-zugeneigte GÜ, 3) überwältigt-verängstigte GÜ, 4) protektiv-elterliche GÜ, 5) desinteressierte GÜ, 6) verstrickte GÜ und 7) sexualisierte GÜ. Die Patienten füllten die Symptomcheckliste (SCL-90R), den Helping Alliance Questionaire (HAQ), das Inventar zur Erfassung interpersonaler Probleme (IIP) und den Fragebogen zur Erhebung von Persönlichkeitsstörungen (ADP-IV) zu Beginn und zum Ende der Therapie aus. Ergebnisse: Es konnten einige spezifische und signifikante Zusammenhänge zwischen der Gegenübertragung der Therapeuten und den Selbstbeurteilungsinstrumenten der Patienten nachgewiesen werden: a) Konfliktorientierte Therapieverfahren (Gesprächstherapien) erzeugen bei den Therapeuten höhere aggressiv-resignative Gegenübertragung, erlebnisorientierte Therapieverfahren (Körpertherapie und Gestaltungstherapie) rufen höhere positiv-zugeneigte, protektiv-elterliche und verstrickte Gegenübertragung hervor. b) Die Beziehungszufriedenheit der Patienten ist
umso größer, je geringer die aggressiv-resignative Gegenübertragung von den Therapeuten wahrgenommen wird und die Therapiezufriedenheit ist umso größer, je geringer die aggressiv-resignative, überwältigt-verängstigte und protektiv-elterliche Gegenübertragung ausgeprägt ist. Ein hoher IIP-Wert bei den Patienten hängt mit dem Erleben von überwältigt-verängstigter Gegenübertragung zusammen. c) Patienten mit hohem GSI rufen bei den Therapeuten hohe aggressiv-resignative, überwältigt-verängstigte, desinteressierte und geringe positiv-zugeneigte Gegenübertragung hervor. Verstrickte, positiv-zugeneigte und sexualisierte Gegenübertragung hängen mit einem guten Therapieverlauf zusammen, aggressiv-resignative und desinteressierte Gegenübertragung deuten auf eine schlechte Entwicklung im Verlauf hin. d) Die höchsten Werte der aggressiv-resignativen, überwältigtverängstigten und protektiv-elterlichen GÜ und die geringste positiv-zugeneigte GÜ wird bei der Gruppe der Persönlichkeitsstörungen wahrgenommen. Den höchsten Wert der positiv-zugeneigten und der verstrickten GÜ erreicht die Gruppe der Essstörungen. Somatoforme Störungen rufen in den Therapeuten ein hohes Maß an Desinteresse hervor. Die Gruppe der affektiven Störungen erzeugt bei den Therapeuten in allen Dimensionen geringe Gegenübertragungsgefühle. Folgerung: Die Ergebnisse verdeutlichen, dass die Gegenübertragung ein aussagekräftiges Instrument zur Beurteilung der Beziehungsgestaltung, der Belastung und der Verlaufsbeurteilung der Patienten auf einer psychosomatischen Station darstellt. Sie betonen auch die Bedeutung der Auflösung negativer Gegenübertragungskonstellationen durch Supervisionen und Teambesprechungen im stationären Alltag. Dabei bedürfen schwierige Patienten, beispielsweise mit somatoformer Störung oder Persönlichkeitsstörung besonderer Aufmerksamkeit. Dem CTQ-D kann neben dem Einsatz als wissenschaftlichem Instrument im Rahmen der Ausbildung und im klinischen Alltag zur Vorhersage des Therapieverlaufs umfassende Bedeutung zukommen.
Introduction. Balapiravir (R1626, RG1626) is the prodrug of a nucleoside analogue inhibitor of the hepatitis C virus (HCV) RNA-dependent RNA polymerase (R1479, RG1479). This phase 2, double-blind international trial evaluated the optimal treatment regimen of balapiravir plus peginterferon alfa-2a (40KD)/ribavirin.
Material and methods. Treatment-naive genotype 1 patients (N = 516) were randomized to one of seven treatment groups in which they received balapiravir 500, 1,000, or 1,500 mg twice daily, peginterferon alfa2a (40KD) 180 or 90 Mg/week and ribavirin 1,000/1,200 mg/day or peginterferon alfa-2a (40KD)/ribavirin. The planned treatment duration with balapiravir was reduced from 24 to 12 weeks due to safety concerns.
Results. The percentage of patients with undetectable HCV RNA was consistently higher in all balapiravir groups from week 2 to 12. However, high rates of dose modifications and discontinuations of one/all study drugs compromised the efficacy assessment and resulted in similar sustained virological response rates in the balapiravir groups (range 32-50%) and the peginterferon alfa-2a (40KD)/ribavirin group (43%). Balapiravir was discontinued for safety reasons in 28-36% of patients (most often for lymphopenia) and the percentage of patients with serious adverse events (especially hematological, infection, ocular events) was dose related. Serious hematological adverse events (particularly neutropenia, lymphopenia) were more common in balapiravir recipients. Two deaths in the balapiravir/peginterferon alfa-2a/ribavirin combination groups were considered possibly related to study medication.
Conclusion. Further development of balapiravir for the treatment of chronic hepatitis C has been halted because of the unacceptable benefit to risk ratio revealed in this study (www.ClinicalTrials.gov NCT 00517439).
Tumor-associated macrophages (TAM) are a major supportive component within neoplasms and by their plasticity promote all phases of tumor development. Mechanisms of macrophage (M Phi) attraction and differentiation to a tumor-promoting phenotype, defined among others by distinct cytokine patterns such as pronounced immunosuppressive interleukin 10 (IL-10) production, are largely unknown. However, a high apoptosis index within tumors and strong M Phi infiltration correlate with poor prognosis. Thus, I aimed at identifying signaling pathways contributing to generation of TAM-like M Phi by using supernatant of apoptotic cancer cells (ACM) as stimulus.
To distinguish novel factors involved in generating TAM-like M Phi, I used an adenoviral RNAi-based approach. The primary read-out was production of IL-10. However, mediators modulating IL-10 were re-validated for their impact on regulation of the cytokines IL-6, IL-8 and IL-12. Following assay development, optimization and down-scaling to a 384-well format, primary human M Phi were transduced with 8495 constructs of the adenoviral shRNA SilenceSelect® library of Galapagos BV, followed by activation to a TAM-like phenotype using ACM. I identified 96 genes involved in IL-10 production in response to ACM and observed a pronounced cluster of 22 targets regulating IL-10 and IL-6. Principal validation of five targets of the IL-10/IL-6 cluster was performed using siRNA or pharmacological inhibitors. Among those, IL-4 receptor-alpha and cannabinoid receptor 2 were confirmed as regulators of IL-10 and IL-6 secretion.
One protein identified in the screen, the nerve growth factor (NGF) receptor TRKA was chosen for in-depth validation, based on its involvement in IL-10, IL-6 and IL-12 secretion from ACM-stimulated human M Phi. TRKA possesses a cardinal role in neuronal development, but compelling evidence emerges suggesting participation of TRKA in cancer development. First experiments using pharmacological inhibitors principally confirmed the involvement of TRKA in IL-10 secretion by ACM-stimulated M Phi and revealed PI3K/AKT and to a lesser extend MAPK p38 as important signaling molecules downstream of TRKA activation. Signaling through TRKA required the presence of its ligand NGF, as indicated by NGF neutralization experiments. NGF was not induced by or present in ACM, but was constitutively secreted by M Phi. Interestingly, M Phi responded to authentic NGF with neither AKT and p38 phosphorylation nor IL-10 production. TRKA is well known to be transactivated by other receptors and in neurons its cellular localization is decisive for its function. Inhibitors of common transactivation partners did not influence IL-10 production by human M Phi. Rather, ACM-treatment provoked pronounced translocation of TRKA to the plasma membrane within 10 minutes as observed by immunofluorescence staining. Consequently, I was intrigued to clarify mechanisms of TRKA trafficking in response to ACM.
The bioactive lipid sphingosine-1-phosphate (S1P) has been previously identified as important apoptotic cell-derived mediator involved in TAM-like M Phi polarization. Indeed, I observed S1P and src kinase involvement in ACM-mediated IL-10 induction. Furthermore, inhibition of S1P receptor (S1PR) signaling or src kinase activity prevented TRKA translocation, whereas a TRKA inhibitor or anti-NGF did not block TRKA trafficking to the plasma membrane in response to ACM. Thus, autocrine secreted NGF activated TRKA to promote IL-10 secretion, which required previous S1PR/src-dependent translocation of TRKA to the plasma membrane. Following the detailed analysis of IL-10 regulation, I was interested whether other TAM phenotype markers were influenced by ACM and whether their expression was regulated through TRKA-dependent signaling. Five of six markers were up-regulated on mRNA level by ACM, and secretion of IL-6, IL-8 and TNF-alpha was triggered. S1PR-signaling was essential for induction of all but one marker, whereas TRKA signaling was only required for cytokine secretion. Interestingly, none of the investigated TAM markers was regulated identically to IL-10, emphasizing a tight and exclusive regulation machinery of this potent immunosuppressive cytokine.
Finally, I aimed to validate the in vitro findings in human ACM-stimulated M Phi. Therefore, I isolated murine TAM as well as other major mononuclear phagocyte populations from primary oncogene-induced breast cancer tissue. Indeed, TRKA-dependent signaling was required for spontaneous cytokine production selectively by primary murine TAM. Besides IL-10, the TRKA pathway was decisive for secretion of IL-6, TNF-alpha and monocyte chemotactic protein-1, indicating its relevance in cancer-associated inflammation.
In summary, my findings highlight a fine-tuned regulatory system of S1P-dependent TRKA trafficking and autocrine NGF signaling in TAM biology. Both factors, S1P as well as NGF, might be interesting targets for future cancer therapy.
