Phenytoin (PHT), valproic acid, and modern antiepileptic drugs (AEDs), eg, remacemide, loreclezole, and safinamide, are only effective within a maximum of 70%-80% of epileptic patients, and in many cases the clinical use of AEDs is restricted by their side effects. Therefore, a continuous need remains to discover innovative chemical entities for the development of active and safer AEDs. Ligands targeting central histamine H3 receptors (H3Rs) for epilepsy might be a promising therapeutic approach. To determine the potential of H3Rs ligands as new AEDs, we recently reported that no anticonvulsant effects were observed for the (S)-2-(4-(3-(piperidin-1-yl)propoxy)benzylamino)propanamide (1). In continuation of our research, we asked whether anticonvulsant differences in activities will be observed for its R-enantiomer, namely, (R)-2-(4-(3-(piperidin-1-yl)propoxy)benzylamino)propaneamide (2) and analogs thereof, in maximum electroshock (MES)-, pentylenetetrazole (PTZ)-, and strychnine (STR)-induced convulsion models in rats having PHT and valproic acid (VPA) as reference AEDs. Unlike the S-enantiomer (1), the results show that animals pretreated intraperitoneally (ip) with the R-enantiomer 2 (10 mg/kg) were moderately protected in MES and STR induced models, whereas proconvulsant effect was observed for the same ligand in PTZ-induced convulsion models. However, animals pretreated with intraperitoneal doses of 5, 10, or 15 mg/kg of structurally bulkier (R)-enantiomer (3), in which 3-piperidinopropan-1-ol in ligand 2 was replaced by (4-(3-(piperidin-1-yl)propoxy)phenyl)methanol, and its (S)-enantiomer (4) significantly and in a dose-dependent manner reduced convulsions or exhibited full protection in MES and PTZ convulsions model, respectively. Interestingly, the protective effects observed for the (R)-enantiomer (3) in MES model were significantly greater than those of the standard H3R inverse agonist/antagonist pitolisant, comparable with those observed for PHT, and reversed when rats were pretreated with the selective H3R agonist R-(α)-methyl-histamine. Comparisons of the observed antagonistic in vitro affinities among the ligands 1-6 revealed profound stereoselectivity at human H3Rs with varying preferences for this receptor subtype. Moreover, the in vivo anticonvulsant effects observed in this study for ligands 1-6 showed stereoselectivity in different convulsion models in male adult rats.
The Asian tiger mosquito, Aedes albopictus (Diptera: Culicidae, SKUSE), is an important threat to public health due to its rapid spread and its potential as a vector. The eggs of Ae. albopictus are the most cold resistant life stage and thus, the cold hardiness of eggs is used to predict the future occurrence of the species in distribution models. However, the mechanism of cold hardiness has yet to be revealed. To address this question, we analyzed the layers of diapausing and cold acclimatized eggs of a temperate population of Ae. albopictus in a full factorial test design using transmission electron microscopy. We reviewed the hypotheses that a thickened wax layer or chorion is the cause of cold hardiness but found no evidence. As a result of the induced diapause, the thickness of the dark endochorion as a layer of high electron density and thus an assumed location for waxes was decreasing. We therefore hypothesized a qualitative alteration of the wax layer due to compaction. Cold acclimation was causing an increase in the thickness of the middle serosa cuticle indicating a detachment of serosa membrane from the endochorion as a potential adaptation strategy to isolate inoculating ice formations in the inter-membranous space.
Sequence type 131 (ST131) is one of the predominant Escherichia coli lineages among extraintestinal pathogenic E. coli (ExPEC) that causes a variety of diseases in humans and animals and frequently shows multidrug resistance. Here, we report the first genome sequence of an ST131-ExPEC strain from poultry carrying the plasmid-encoded colistin resistance gene mcr-1.
Zielsetzung. Diese Pilotstudie untersucht den therapeutischen Effekt des RelaxBogens in Bezug auf Bruxismus und Symptome einer kraniomandibulären Dysfunktion (CMD).
Probanden und Methoden. Untersucht wurde eine Gruppe von 10 Probanden. Auswahlkriterien waren ein vorliegender Bruxismus und erste Symptome einer CMD. Dies wurde durch die Anwendung einer Brux-Checker®-Folie für 2 Nächte und eine Schmerzanamnese verifiziert. Eingangs wurden neben einer ausführlichen zahnärztlichen Anamnese folgende CMDParameter erhoben: SL-NRS-Fragebogen sowie Palpation von 42 Muskeln im Kopf-HalsNacken-Bereich und der 6 Austrittspunkte des N. trigeminus. Nach der Voruntersuchung wurde der RelaxBogen mindestens 10 Wochen getragen. Danach wurden die genannten Parameter erneut erhoben und miteinander verglichen.
Ergebnisse. Die Vor- und Nachuntersuchungen zeigten starke Tendenzen der Reduktion sowohl der allgemeinen Symptomatik als auch der Schmerzempfindung im Kieferbereich. Nach dem Tragen des RelaxBogens konnte eine eindeutige Reduktion des durch Palpation ausgelösten, muskulären Schmerzes beobachtet werden. Ebenfalls deutlich positive Tendenzen ließen sich bei der Verbesserung des allgemeinen Wohlbefindens finden. Die Ergebnisse legen nahe, dass bei einer größeren Stichprobe eine statistische Signifikanz zu erwarten ist.
Schlussfolgerung. Der RelaxBogen führt nach den Ergebnissen dieser Pilotstudie bei Patienten, die unter Bruxismus- und CMD-Symptomen leiden, zu einer deutlichen Reduktion der Schmerzwahrnehmung sowie einer eindeutigen Steigerung des Wohlbefindens. Neben der Verringerung des Schmerzempfindens und einem reduzierten Spannungsgefühl in den großen Kiefermuskeln konnten auch positive Tendenzen im Bereich der Hals-, Nacken- und Schultermuskulatur festgestellt werden.
Da HRS-Zellen im cHL nur eine Minderheit und CD4+ T-Zellen die Mehrheit im Begleitinfiltrat ausmachen, wurde innerhalb der vorliegenden Dissertation das Begleitinfiltrat und der Tumorzellgehalt von 24 HIV-assoziierten cHL-Fällen mit 15 HIV-negativen cHL-Fällen immunhistochemisch verglichen. Das reaktive Begleitinfiltrat im HIV-assoziierten cHL zeigte eine deutlich geringere Anzahl an CD4+ T-Zellen und einen höheren Gehalt an CD163+ Makrophagen als das HIV-negative cHL. Es konnte kein Unterschied in der Anzahl der CD30+ HRS-Zellen und S100+ dendritischen Zellen zwischen beiden Gruppen festgestellt werden. Mit Kokultur-Versuchen im Labor und darauf folgenden Zellausstrichen dieser Kokulturen konnte bestätigt werden, dass sich CD14+ Monozyten ebenso gut wie CD4+ T-Zellen als Rosetten um HRS-Zellen anordnen können. Im immunkomprimierten HIV-Patienten ersetzen die langlebigen CD163+ Makrophagen die CD4+ T-Zellen. Die Makrophagen werden vermutlich ebenso wie CD4+ T-Zellen mittels Zytokine/Chemokine (z. B. CCL5) zum Tumorgewebe rekrutiert, bilden Rosetten um die Tumorzellen und unterstützen diese in ihrer Proliferation.
Aufgrund der besonderen Zusammensetzung des Begleitinfiltrats sollte das HIV-assoziierte cHL von Pathologen als eigenständiger Subtyp des cHL betrachtet werden.
Des Weiteren wurde das Begleitinfiltrat der typisch knotigen NLPHL Typen A und C mit dem des diffusen NLPHL Typen E (THRLBCL-like NLPHL) und dem THRLBCL immunhistochemisch verglichen. Aufgrund histologischer und klinischer Ähnlichkeiten zwischen dem diffusen NLPHL und dem THRLBCL fällt eine Differenzierung dieser Entitäten schwer. Es konnte festgestellt werden, dass das Begleitinfiltrat im THRLBCL-like NLPHL dem Begleitinfiltrat im THRLBCL mehr ähnelt als dem typischen NLPHL und zwar in Bezug auf Makrophagengehalt und Anzahl der follikulären TFH-Zellen. Es konnten Rosetten im Begleitinfiltrat von THRLBCL nachgewiesen werden, obwohl Rosettenformationen um Tumorzellen im THRLBCL in der Literatur kein charakteristisches Merkmal darstellen. Es ist naheliegend, dass das THRLBCL-like NLPHL und das THRLBCL ein und dieselbe Krankheit ist und möglicherweise eine aggressivere Variante des NLPHL darstellt.
Im Anbetracht aller Ergebnisse kommt dem Immunstatus eines Patienten eine ausschlaggebende Rolle auf das Begleitinfiltrat im Tumorgewebe zu und dieser beeinflusst so auch den klinischen Verlauf der Lymphomerkrankung.
OBJECTIVES: Identification of sufficiently trustworthy top 5 list recommendations from the US Choosing Wisely campaign.
SETTING: Not applicable.
PARTICIPANTS: All top 5 list recommendations available from the American Board of Internal Medicine Foundation website.
MAIN OUTCOME MEASURES/INTERVENTIONS: Compilation of US top 5 lists and search for current German highly trustworthy (S3) guidelines. Extraction of guideline recommendations, including grade of recommendation (GoR), for suggestions comparable to top 5 list recommendations. For recommendations without guideline equivalents, the methodological quality of the top 5 list development process was assessed using criteria similar to that used to judge guidelines, and relevant meta-literature was identified in cited references. Judgement of sufficient trustworthiness of top 5 list recommendations was based either on an 'A' GoR of guideline equivalents or on high methodological quality and citation of relevant meta-literature.
RESULTS: 412 top 5 list recommendations were identified. For 75 (18%), equivalents were found in current German S3 guidelines. 44 of these recommendations were associated with an 'A' GoR, or a strong recommendation based on strong evidence, and 26 had a 'B' or a 'C' GoR. No GoR was provided for 5 recommendations. 337 recommendations had no equivalent in the German S3 guidelines. The methodological quality of the development process was high and relevant meta-literature was cited for 87 top 5 list recommendations. For a further 36, either the methodological quality was high without any meta-literature citations or meta-literature citations existed but the methodological quality was lacking. For the remaining 214 recommendations, either the methodological quality was lacking and no literature was cited or the methodological quality was generally unsatisfactory.
CONCLUSIONS: 131 of current US top 5 list recommendations were found to be sufficiently trustworthy. For a substantial number of current US top 5 list recommendations, their trustworthiness remains unclear. Methodological requirements for developing top 5 lists are recommended.
Background: Arising from the relevance of sensorimotor training in the therapy of nonspecific low back pain patients and from the value of individualized therapy, the present trial aims to test the feasibility and efficacy of individualized sensorimotor training interventions in patients suffering from nonspecific low back pain.
Methods and study design: A multicentre, single-blind two-armed randomized controlled trial to evaluate the effects of a 12-week (3 weeks supervised centre-based and 9 weeks home-based) individualized sensorimotor exercise program is performed. The control group stays inactive during this period. Outcomes are pain, and pain-associated function as well as motor function in adults with nonspecific low back pain. Each participant is scheduled to five measurement dates: baseline (M1), following centre-based training (M2), following home-based training (M3) and at two follow-up time points 6 months (M4) and 12 months (M5) after M1. All investigations and the assessment of the primary and secondary outcomes are performed in a standardized order: questionnaires – clinical examination – biomechanics (motor function). Subsequent statistical procedures are executed after the examination of underlying assumptions for parametric or rather non-parametric testing.
Discussion: The results and practical relevance of the study will be of clinical and practical relevance not only for researchers and policy makers but also for the general population suffering from nonspecific low back pain.
Recently, the conserved intracellular digestion mechanism ‘autophagy’ has been considered to be involved in early tumorigenesis and its blockade proposed as an alternative treatment approach. However, there is an ongoing debate about whether blocking autophagy has positive or negative effects in tumor cells. Since there is only poor data about the clinico-pathological relevance of autophagy in gliomas in vivo, we first established a cell culture based platform for the in vivo detection of the autophago-lysosomal components. We then investigated key autophagosomal (LC3B, p62, BAG3, Beclin1) and lysosomal (CTSB, LAMP2) molecules in 350 gliomas using immunohistochemistry, immunofluorescence, immunoblotting and qPCR. Autophagy was induced pharmacologically or by altering oxygen and nutrient levels. Our results show that autophagy is enhanced in astrocytomas as compared to normal CNS tissue, but largely independent from the WHO grade and patient survival. A strong upregulation of LC3B, p62, LAMP2 and CTSB was detected in perinecrotic areas in glioblastomas suggesting micro-environmental changes as a driver of autophagy induction in gliomas. Furthermore, glucose restriction induced autophagy in a concentration-dependent manner while hypoxia or amino acid starvation had considerably lesser effects. Apoptosis and autophagy were separately induced in glioma cells both in vitro and in vivo. In conclusion, our findings indicate that autophagy in gliomas is rather driven by micro-environmental changes than by primary glioma-intrinsic features thus challenging the concept of exploitation of the autophago-lysosomal network (ALN) as a treatment approach in gliomas.
Multimodal therapy of glioblastoma (GBM) reveals inter-individual variability in terms of treatment outcome. Here, we examined whether a miRNA signature can be defined for the a priori identification of patients with particularly poor prognosis.
FFPE sections from 36 GBM patients along with overall survival follow-up were collected retrospectively and subjected to miRNA signature identification from microarray data. A risk score based on the expression of the signature miRNAs and cox-proportional hazard coefficients was calculated for each patient followed by validation in a matched GBM subset of TCGA. Genes potentially regulated by the signature miRNAs were identified by a correlation approach followed by pathway analysis.
A prognostic 4-miRNA signature, independent of MGMT promoter methylation, age, and sex, was identified and a risk score was assigned to each patient that allowed defining two groups significantly differing in prognosis (p-value: 0.0001, median survival: 10.6 months and 15.1 months, hazard ratio = 3.8). The signature was technically validated by qRT-PCR and independently validated in an age- and sex-matched subset of standard-of-care treated patients of the TCGA GBM cohort (n=58). Pathway analysis suggested tumorigenesis-associated processes such as immune response, extracellular matrix organization, axon guidance, signalling by NGF, GPCR and Wnt. Here, we describe the identification and independent validation of a 4-miRNA signature that allows stratification of GBM patients into different prognostic groups in combination with one defined threshold and set of coefficients that could be utilized as diagnostic tool to identify GBM patients for improved and/or alternative treatment approaches.
Ligand stimulation of CD95 induces activation of Plk3 followed by phosphorylation of caspase-8

Upon interaction of the CD95 receptor with its ligand, sequential association of the adaptor molecule FADD (MORT1), pro-forms of caspases-8/10, and the caspase-8/10 regulator c-FLIP leads to the formation of a death-inducing signaling complex. Here, we identify polo-like kinase (Plk) 3 as a new interaction partner of the death receptor CD95. The enzymatic activity of Plk3 increases following interaction of the CD95 receptor with its ligand. Knockout (KO) or knockdown of caspase-8, CD95 or FADD prevents activation of Plk3 upon CD95 stimulation, suggesting a requirement of a functional DISC for Plk3 activation. Furthermore, we identify caspase-8 as a new substrate for Plk3. Phosphorylation occurs on T273 and results in stimulation of caspase-8 proapoptotic function. Stimulation of CD95 in cells expressing a non-phosphorylatable caspase-8-T273A mutant in a rescue experiment or in Plk3-KO cells generated by CRISPR/Cas9 reduces the processing of caspase-8 prominently. Low T273 phosphorylation correlates significantly with low Plk3 expression in a cohort of 95 anal tumor patients. Our data suggest a novel mechanism of kinase activation within the Plk family and propose a new model for the stimulation of the extrinsic death pathway in tumors with high Plk3 expression.
