A case of Lymphangioleiomyomatosis (LAM) of the lung in a patient with a history of breast cancer

Background: Lymphangioleiomyomatosis (LAM) is a rare progressive cystic and nodular disease of the lung characterized by smooth muscle cell proliferation. LAM predominantly affects young premenopausal women. This report is of a case of LAM presenting in a 47-year-old woman with a past history of breast cancer and discusses the possibility of an association between the two conditions.
Case report: A 47-year-old woman presented as an emergency with an exacerbation of a four-month history of shortness of breath and dry cough. Her symptoms began following the start of anti-hormonal treatment with letrozole and goserelin acetate for a moderately differentiated (grade 2) invasive ductal carcinoma of the breast (pT2, pN0, M0) which was positive for expression of estrogen receptor (ER+), progesterone receptor (PR+), and human epidermal growth factor receptor 2 (HER2+). Until the previous four months, she had breast-conserving treatment with radiotherapy and tamoxifen therapy. Following hospital admission, she was found to be in type I respiratory failure. Chest X-ray, lung computed tomography (CT), and positron-emission tomography (PET) showed diffuse cystic and nodular lung lesions, consistent with a diagnosis of LAM, and antihormonal therapy was discontinued. She developed pericarditis that was treated with the anti-inflammatory agent, colchicine. Treatment with letrozole and sirolimus improved her respiratory symptoms.
Conclusions: A rare case of LAM is presented in a woman with a recent history of breast cancer. Because both tumors were hormone-dependent, this may support common underlying gene associations and signaling pathways between the two types of tumor.
Das Adenokarzinom des Pankreas ist das fünfthäufigste Malignom der westlichen Länder mit einer sehr schlechten Prognose. Zum Zeitpunkt der Diagnosestellung ist das Pankreaskarzinom wegen der frühen lokalen Infiltration und Metastasierung meist nicht mehr kurativ behandelbar. 80 % aller Pankreaskarzinome werden in einem fortgeschrittenen Krankheitsstadium diagnostiziert. Die Therapie des fortgeschrittenen Pankreaskarzinoms gestaltet sich problematisch. Bislang führten systemische chemotherapeutische Ansätze nicht zur erhofften Verlängerung der Lebenszeit. In der vorliegenden Phase II Studie wurden in der Abteilung für Allgemein- und Gefäßchirurgie der Universitätsklinik Frankfurt am Main 17 Patienten mit lokal fortgeschrittenem oder metastasiertem Pankreaskarzinom mit einer regional/systemischen Kombinationschemotherapie behandelt. Ein Therapiezyklus bestand aus einer 3ominütigen intraarteriellen Infusion von 8,5 mg/m2 Mitomycin C und aus einer 60minütigen intraarteriellen Infusion von 500 mg/m² Gemcitabine über einen transfemoralen Truncuskatheter an den Tagen 1 und 22, gefolgt von einer systemischen lnfusion von 500 mg/rn² Gemcitabine über 30 Minuten an den Tagen 8, 15, 29 und 36. Die Komhinationschemotherapie war nebenwirkungs- und komplikationsarm. Schwere Nebenwirkungen im NCI-Stadium III/IV wurden im Verlauf von 37 Therapiezyklen mit 74 regionalen Applikationen und 148 systemischen Applikationen in 9 % der Applikationen als Beeinträchtigung der Knochenmarksfunktion (Leukopenie, Thrombopenie und Hämoglobinabfall) und in 15 % der Applikationen als Leberfunktionsstörungen (Erhöhung von Transaminasen, alkalischer Phosphatase und Bilirubin) beobachtet. Die Nebenwirkungen nach regionalen Therapien waren mit den Nebenwirkungen nach venösen Therapien vergleichbar. Kein Patient verstarb an den Folgen einer Nebenwirkung. Ein Patient erlitt nach einer regionalen Chemotherapie einen kompletten Verschluß der Arteria iliaca externa sinistra. Der Therapieerfolg wurde anhand von Computertomographien (CT) und Tumormarker CA 19-9 Bestimmungen nach jedem Chemotherapiezyklus beobachtet und gemäß den WHO-Kriterien beurteilt. Nach CT-Kriterien zeigten vier Patienten eine Regression. Eine komplette Remission wurde bei einer Patientin, eine partielle Remission bei drei Patienten beobachtet. Eine radiologische Remissionsrate von 24 % konnte errechnet werden. Bei fünf Patienten ließ sich unter Therapie keine Größenzunahme des Tumors erkennen (Stable disease). Das Tumorwachstum war bei neun Patienten progredient. Bei sieben Patienten konnte unter Therapie ein Tumormarkerrückgang von mehr als 50 % evaluiert werden (Remissionsrate 41%). Insgesamt zeigten sich zwei komplette Remissionen mit Sinken des Tumormarkers CA 19-9 in den Referenzbereich, d.h. unter 37 µg/ml, und fünf partielle Remissionen. Bei fünf Patienten war der Verlauf des Tumormarkers CA 19-9 unter Therapie stabil (Stable disease). Der Tumormarker CA 19-9 stieg progredient bei fünf Patienten. Das mediane Überleben nach der Kombinationschemotherapie betrug 9,1 Monate (95 % CI: 6-12 Monate). Das mediane Überleben war für Patienten ohne Fernmetastasen (n = 7) mit 15 Monaten (95 % CI: 3-23 Monate) signifikant (p = 0,037) länger als für Patienten mit Fernmetastasen (n = 10) mit 6,3 Monaten (95 % CI: 4,6-12 Monate). Die mediane progressionsfreie Zeit während der Kombinationschemotherapie betrug 4,6 Monate (95 % CI: 2,1-8,7 Monate). Das radiologische Ansprechen (24 %) und die mediane Überlebenszeit (9,1 Monate) dieser regional/systemischen Kombinationschemotherapie waren im Vergleich zu systemischen Standardchemotherapien mit Gemcitabine, die radiologische Remissionsraten von 6,3 % bis 12 % und mediane Überlebenszeiten von 4,8 bis 6,6 Monaten zeigten, erhöht. Die Studie wird aufgrund ihres hohen klinischen Nutzens weiter fortgesetzt. Eine randomisierte Phase III Studie, die die vorliegende regional/systemische Kombinationschemotherapie (Mitomycin C 8,5 mg/m2 Gemcitabine 500 mg/m2) mit dem systemischen Standardverfahren (Gemcitabine 1000 mg/m2) vergleicht, wird angestrebt.
Plasma fibronectin is a circulating protein that facilitates phagocytosis by connecting bacteria to immune cells. A fibronectin isoform, which includes a sequence of 90 AA called extra-domain B (EDB), is synthesized de novo at the messenger RNA (mRNA) level in immune cells, but the reason for its expression remains elusive. We detected an 80-fold increase in EDB-containing fibronectin in the cerebrospinal fluid of patients with bacterial meningitis that was most pronounced in staphylococcal infections. A role for this isoform in phagocytosis was further suggested by enhanced EDB fibronectin release after internalization of Staphylococcus aureus in vitro. Using transgenic mouse models, we established that immune cell production of fibronectin contributes to phagocytosis, more so than circulating plasma fibronectin, and that accentuated release of EDB-containing fibronectin by immune cells improved phagocytosis. In line with this, administration of EDB fibronectin enhanced in vitro phagocytosis to a larger extent than plasma fibronectin. This enhancement was mediated by αvβ3 integrin as shown using inhibitors or cells from β3 integrin knockout mice. Thus, we identified both a novel function for EDB fibronectin in augmenting phagocytosis over circulating plasma fibronectin, as well as the mediating receptor. Our data also establish for the first time, a direct role for β3 integrin in bacterial phagocytosis in mammals.
Angesichts der Bedeutung, die Brustkrebs sowohl in der Medizin als auch im gesellschaftlichen Bewusstsein besitzt, wird es zunehmend wichtiger, ein verstärktes Augenmerk auf die Möglichkeiten und Grenzen entsprechender Vorsorge- und Früherkennungsmaßnahmen zu legen. Seit Mitte der 90er Jahre hat sich das Spektrum der Brustkrebsdiagnostik um die Möglichkeit der genetischen Testung von Risikopatienten erweitert. Der hier angesprochene BRCA-Gentest für erblichen Brust- und Eierstockkrebs zählt zu den prädiktiv-probabilistischen Gentests mit geringer Aussagekraft bezüglich der Vorhersage einer möglichen Krebserkrankung. Weder der genaue Zeitpunkt eines Krankheitsausbruchs, noch die Schwere einer möglichen Erkrankung können genau vorhergesagt werden. Auch ein negatives Testergebnis gibt keinerlei Gewissheit darüber, dass Brust- oder Eierstockkrebs nicht doch im Rahmen des durchschnittlichen Risikos von ca. 8% bei den Getesteten entstehen kann. Es besteht die Gefahr, dass das negative Testergebnis zu einer Unterschätzung dieses durchschnittlichen Risikos führt und entsprechende präventive Maßnahmen nicht ausreichend wahrgenommen werden. Ein positives Testergebnis kann zu Einschränkungen in der Lebensführung und in der weiteren Lebens- und Familienplanung führen. Des Weiteren können prophylaktische Maßnahmen, wie Mastektomie oder Ovarektomie, ergriffen werden, die der Situation möglicherweise nicht angemessen sind und einen fraglichen Nutzen besitzen. Im Zuge der Möglichkeiten einer genetischen Testung ist es zu einer Veränderung bzw. zu einer Erweiterung des Krankheitsbegriffs in der Medizin gekommen. Dabei sei angemerkt, dass die meisten Personen, die eine prädiktive Medizin in Anspruch nehmen, zumeist primär nicht krank sind, sondern eine Aufklärung über eine mögliche genetische Disposition haben möchten. Prädiktive Gentests, die im Falle einer Verdachtsdiagnose in präventive und prophylaktische Maßnahmen münden, setzen zum einen den Noch-Nicht-Kranken einem Behandlungsrisiko aus und machen zum anderen einen bestimmten Anteil an positiv Getesteten zu Patienten, obwohl diese die Erkrankung erst spät oder nie bekommen würden. Besteht bei der Erweiterung des Krankheitsbegriffs nicht die Gefahr der sozialen Diskriminierung von Gesunden mit genetischer Disposition? Ist das System der genetischen Testung für jeden Einzelnen frei zugänglich, und wie lassen sich die zusätzlichen Kosten rechtfertigen? Der Gentest auf erblichen Brust- und Eierstockkrebs ist eingebunden in eine interdisziplinäre Zusammenarbeit aus Gynäkologen, Humangenetikern, Onkologen, Psychoonkologen, Pathologen, Radiologen und Hausärzten. Die Autoren Günter Feuerstein und Regine Kollek kommen jedoch zu dem Schluss, dass auch eine qualifizierte genetische Beratung nicht in der Lage sei, alle Unsicherheiten auszuräumen, die sowohl mit den naturwissenschaftlich-genetischen Risikoaussagen als auch mit deren individuellen Wahrnehmung und Bewertung verbunden sind. Sie stellen sich die Frage, was ein „informed consent“ vor diesem Hintergrund komplexer genetischer Sachlagen und der gegenüber vielfältigen Einflüssen und Faktoren anfälligen Vermittlung probabilistischer Risikoinformationen bedeuten könne. Es stellt sich die Frage, wie es um das Recht auf Nichtwissen und die Freiwilligkeit der Inanspruchnahme von Gentests bestellt ist, wenn eine entsprechende gesundheitliche Verantwortung des Einzelnen gesellschaftlich eingefordert wird und wie viel Verantwortung eine Gesellschaft für den Einzelnen übernehmen kann. Vor diesem Hintergrund erscheint es daher umso wichtiger, einen Wirksamkeitsnachweis der präventiven Maßnahmen und der prophylaktischen Eingriffe zu erbringen. Mehrere Studien haben die Wirksamkeit prophylaktischer Eingriffe in Bezug auf erblichen Brust- und Eierstockkrebs untersucht und kamen zu einer Risikoreduktion für Brustkrebs bei einer prophylaktischen Mastektomie von bis zu 97% und zu einer Risikoreduktion für Eierstockkrebs bei einer prophylaktischen Ovarektomie von bis zu 50%. Ein unbestimmtes Restrisiko bleibt bei jeder Form des prophylaktischen Eingriffs bestehen. Mit entscheidend ist dabei einerseits das subjektive Körpergefühl der betroffenen Frau und andererseits der kulturelle Hintergrund, wie ein Vergleich zwischen Frankreich, Niederlande, England, Kanada, den USA und Deutschland zeigt. Studien und Erfahrungsberichte haben gezeigt, dass individuell auf die Wünsche der Patientinnen eingegangen wird, sie werden in ihren eigenen Vorstellungen bezüglich Körperbild und Vorsorgemaßnahmen respektiert, und besitzen eine aktive Rolle in der Kontrolle ihrer Behandlung. Daher sind sowohl die medizinischen Fachkräfte als auch die Betroffenen aufgefordert, entsprechende Vorsorge- und Früherkennungsmaßnahmen, insbesondere genetische Testverfahren, gemeinsam weiter zu entwickeln, aber auch kritisch zu hinterfragen, um gleichermaßen das Verantwortungsbewusstsein und den Aufklärungsbedarf unter Beachtung individueller Gegebenheiten zu stärken.
Evading innate immunity is a prerequisite for pathogenic microorganisms in order to survive in their respective hosts. Concerning Lyme disease spirochetes belonging to the Borrelia (B.) burgdorferi sensu lato group, a broad range of diverse vertebrates serve as reservoir or even as incidental hosts, including humans. The capability to infect multiple hosts implies that spirochetes have developed sophisticated means to counter the destructive effects of complement of humans and various animals. While the means by which spirochetes overcome the hosts immune defense are far from being completely understood, there is a growing body of evidence suggesting that binding of the key regulator of the alternative pathway, Factor H, plays a pivotal role for immune evasion and that Factor H is an important determinant of host specificity. This review covers (i) the contribution of complement in host-specificity and transmissibility of Lyme disease spirochetes; (ii) the involvement of borrelial-derived determinants to host specificity; (iii) the interplay of human and animal Factor H with complement-acquiring surface proteins of diverse borrelial species; and (iv) the potential role of additional animal complement proteins in the immune evasion of spirochetes.
The present study investigates the hemispheric contributions of neuronal reorganization following early single-sided hearing (unilateral deafness). The experiments were performed on ten cats from our colony of deaf white cats. Two were identified in early hearing screening as unilaterally congenitally deaf. The remaining eight were bilaterally congenitally deaf, unilaterally implanted at different ages with a cochlear implant. Implanted animals were chronically stimulated using a single-channel portable signal processor for two to five months. Microelectrode recordings were performed at the primary auditory cortex under stimulation at the hearing and deaf ear with bilateral cochlear implants. Local field potentials (LFPs) were compared at the cortex ipsilateral and contralateral to the hearing ear. The focus of the study was on the morphology and the onset latency of the LFPs. With respect to morphology of LFPs, pronounced hemisphere-specific effects were observed. Morphology of amplitude-normalized LFPs for stimulation of the deaf and the hearing ear was similar for responses recorded at the same hemisphere. However, when comparisons were performed between the hemispheres, the morphology was more dissimilar even though the same ear was stimulated. This demonstrates hemispheric specificity of some cortical adaptations irrespective of the ear stimulated. The results suggest a specific adaptation process at the hemisphere ipsilateral to the hearing ear, involving specific (down-regulated inhibitory) mechanisms not found in the contralateral hemisphere. Finally, onset latencies revealed that the sensitive period for the cortex ipsilateral to the hearing ear is shorter than that for the contralateral cortex. Unilateral hearing experience leads to a functionally-asymmetric brain with different neuronal reorganizations and different sensitive periods involved.
Context: Autoimmune polyglandular syndrome (APS-2: autoimmune Addison’s disease or type 1 diabetes) is conferred by predisposing HLA molecules, vitamin D deficiency, and heritable susceptibility. Organ destruction is accompanied by cytokine alterations. We addressed the monocytic cytokines of two distinct APS-2 cohorts, effects of vitamin D and HLA DQ risk.
Methods: APS-2 patients (n = 30) and healthy controls (n = 30) were genotyped for HLA DQA1/DQB1 and their CD14+ monocytes stimulated with IL1β and/or 1,25(OH)2D3 for 24 h. Immune regulatory molecules (IL-6, IL-10, IL-23A, IL-15, CCL-2, PD-L1), vitamin D pathway gene transcripts (CYP24A1, CYP27B1, VDR), and CD14 were analyzed by enzyme-linked immunosorbent assay and RTqPCR. Results: Pro-inflammatory CCL-2 was higher in APS-2 patients than in controls (p = 0.001), whereas IL-6 showed a trend – (p = 0.1). In vitro treatment with 1,25(OH)2D3 reduced proinflammatory cytokines (IL-6, CCL-2, IL-23A, IL-15) whereas anti-inflammatory cytokines (IL-10 and PD-L1) rose both in APS-type 1 diabetes and APS-Addison´s disease. Patients with adrenal autoimmunity showed a stronger response to vitamin D. Expression of IL-23A and vitamin D pathway genes VDR and CYP27B1 varied by HLA genotype and was lower in healthy individuals with high-risk HLA (p = 0.0025; p = 0.04), while healthy controls with low-risk HLA showed a stronger IL-10 and CD14 expression (p = 0.01; p = 0.03). Conclusion: 1,25(OH)2D3 regulates the monocytic response in APS-2 disorders type 1 diabetes or Addison´s disease. The monocytic cytokine profile of individuals carrying HLA high-risk alleles is proinflammatory, enhances polyglandular autoimmunity and can be targeted by vitamin D.
Human endogenous retrovirus (HERV) genomes are chromosomally integrated in all cells of an individual. They are normally transcriptionally silenced and transmitted only vertically. Enhanced expression of HERV-K accompanied by the emergence of anti-HERV-K-directed immune responses has been observed in tumor patients and HIV-infected individuals. As HERV-K is usually not expressed and immunological tolerance development is unlikely, it is an appropriate target for the development of immunotherapies. We generated a recombinant vaccinia virus (MVA-HKenv) expressing the HERV-K envelope glycoprotein (ENV), based on the modified vaccinia virus Ankara (MVA), and established an animal model to test its vaccination efficacy. Murine renal carcinoma cells (Renca) were genetically altered to express E. coli beta-galactosidase (RLZ cells) or the HERV-K ENV gene (RLZ-HKenv cells). Intravenous injection of RLZ-HKenv cells into syngenic BALB/c mice led to the formation of pulmonary metastases, which were detectable by X-gal staining. A single vaccination of tumor-bearing mice with MVA-HKenv drastically reduced the number of pulmonary RLZ-HKenv tumor nodules compared to vaccination with wild-type MVA. Prophylactic vaccination of mice with MVA-HKenv precluded the formation of RLZ-HKenv tumor nodules, whereas wild-type MVA-vaccinated animals succumbed to metastasis. Protection from tumor formation correlated with enhanced HERV-K ENV-specific killing activity of splenocytes. These data demonstrate for the first time that HERV-K ENV is a useful target for vaccine development and might offer new treatment opportunities for diverse types of cancer.
Background: Patients with chronic kidney disease (CKD) are at high risk of myocardial infarction. Cardiac troponins are the biomarkers of choice for the diagnosis of acute myocardial infarction (AMI) without ST‐segment elevation (NSTE). In patients with CKD, troponin levels are often chronically elevated, which reduces their diagnostic utility when NSTE‐AMI is suspected. The aim of this study was to derive a diagnostic algorithm for serial troponin measurements in patients with CKD and suspected NSTE‐AMI.
Methods and Results: Two cohorts, 1494 patients from a prospective cohort study with high‐sensitivity troponin I (hs‐cTnI) measurements and 7059 cases from a clinical registry with high‐sensitivity troponin T (hs‐cTnT ) measurements, were analyzed. The prospective cohort comprised 280 CKD patients (estimated glomerular filtration rate <60 mL/min/1.73 m2). The registry data set contained 1581 CKD patients. In both cohorts, CKD patients were more likely to have adjudicated NSTE‐AMI than non‐CKD patients. The specificities of hs‐cTnI and hs‐cTnT to detect NSTE‐AMI were reduced with CKD (0.82 versus 0.91 for hs‐cTnI and 0.26 versus 0.73 for hs‐cTnT) but could be restored by applying optimized cutoffs to either the first or a second measurement after 3 hours. The best diagnostic performance was achieved with an algorithm that incorporates serial measurements and rules in or out AMI in 69% (hs‐cTnI) and 55% (hs‐cTnT) of CKD patients.
Conclusions: The diagnostic performance of high‐sensitivity cardiac troponins in patients with CKD with suspected NSTE‐AMI is improved by use of an algorithm based on admission troponin and dynamic changes in troponin concentration.
