Purpose: Subcutaneous allergen-specific immunotherapy (SCIT) is a well-established and clinically effective method to treat allergic diseases, such as rhinitis and asthma. It remains unclear how soon after initiation of an ultra-short course of grass pollen immunotherapy adjuvanted with monophosphoryl lipid A (MPL)-specific bronchial tolerance can be induced.
Methods: In a prospective study of 69 children double-sensitized to birch and grass pollens (51 males, average age 11.1 years), development of bronchial tolerance after 1 cycle of SCIT for grass was evaluated. In all the patients, the bronchial allergen provocation test (BAP) was performed before and after treatment. According to the results of the first BAP, the patients were divided into 2 groups: those showing a negative BAP with a decrease in FEV1 of <20% (seasonal allergic rhinitis [SAR] group, n=47); and those showing a positive BAP with a decrease in FEV1 of ≥20% (SAR with allergic asthma [SAR and Asthma] group, n=22). All the patients received MPL-adjuvanted, ultra-short course immunotherapy for birch, but only those with a positive BAP to grass received MPL-SCIT for grass.
Results: After the pollen season, the BAP in the SAR group remained unchanged, while it was improved in the SAR and Asthma group (decrease in FEV1 of 28.8% vs 12.5%, P<0.01). The IgG4 levels increased after SCIT (median before SCIT 0.34 to 11.4 after SCIT), whereas the total and specific IgE levels remained unchanged.
Conclusions: After 1 cycle of MPL-SCIT, specific bronchial tolerance may be significantly induced, whereas in patients without SCIT, bronchial hyperactivity may remain unchanged.
Treatment of malignant glioma with chemotherapy is limited mostly because of delivery impediment related to the blood-brain tumor barrier (BTB). B1 receptors (B1R), inducible prototypical G-protein coupled receptors (GPCR) can regulate permeability of vessels including possibly that of brain tumors. Here, we determine the extent of BTB permeability induced by the natural and synthetic peptide B1R agonists, LysdesArg9BK (LDBK) and SarLys[DPhe8]desArg9BK (NG29), in syngeneic F98 glioma-implanted Fischer rats. Ten days after tumor inoculation, we detected the presence of B1R on tumor cells and associated vasculature. NG29 infusion increased brain distribution volume and uptake profiles of paramagnetic probes (Magnevist and Gadomer) at tumoral sites (T1-weighted imaging). These effects were blocked by B1R antagonist and non-selective cyclooxygenase inhibitors, but not by B2R antagonist and non-selective nitric oxide synthase inhibitors. Consistent with MRI data, systemic co-administration of NG29 improved brain tumor delivery of Carboplatin chemotherapy (ICP-Mass spectrometry). We also detected elevated B1R expression in clinical samples of high-grade glioma. Our results documented a novel GPCR-signaling mechanism for promoting transient BTB disruption, involving activation of B1R and ensuing production of COX metabolites. They also underlined the potential value of synthetic biostable B1R agonists as selective BTB modulators for local delivery of different sized-therapeutics at (peri)tumoral sites.
Background: In Emergency and Medical Admission Departments (EDs and MADs), prompt recognition and appropriate infection control management of patients with Highly Infectious Diseases (HIDs, e.g. Viral Hemorrhagic Fevers and SARS) are fundamental for avoiding nosocomial outbreaks.
Methods: The EuroNHID (European Network for Highly Infectious Diseases) project collected data from 41 EDs and MADs in 14 European countries, located in the same facility as a national/regional referral centre for HIDs, using specifically developed checklists, during on-site visits from February to November 2009.
Results: Isolation rooms were available in 34 facilities (82,9%): these rooms had anteroom in 19, dedicated entrance in 15, negative pressure in 17, and HEPA filtration of exhausting air in 12. Only 6 centres (14,6%) had isolation rooms with all characteristics. Personnel trained for the recognition of HIDs was available in 24 facilities; management protocols for HIDs were available in 35.
Conclusions: Preparedness level for the safe and appropriate management of HIDs is partially adequate in the surveyed EDs and MADs.
Infections are an important cause for morbidity and mortality in pediatric acute myeloid leukemia (AML). We therefore characterized infectious complications in children treated according to the trial AML-BFM 2004. Patients with Down syndrome were excluded from the analysis. Data were gathered from the medical records in the hospital where the patients were treated. A total of 405 patients (203 girls; median age 8.4 years) experienced 1326 infections. Fever without identifiable source occurred in 56.1% of the patients and clinically and microbiologically documented infections in 17.5% and 32.4% of the patients, respectively. In all, 240 Gram-positive (112 viridans group streptococci) and 90 Gram-negative isolates were recovered from the bloodstream. Invasive fungal infection was diagnosed in 3% of the patients. Three children each died of Gram-negative bacteremia and invasive aspergillosis, respectively. As compared with the results of AML-BFM 93 with lower dose intensity, infection-related morbidity was slightly higher in AML-BFM 2004 (3.3. versus 2.8 infections per patient), whereas infection-related mortality significantly decreased (1.5% versus 5.4%; P=0.003). Specific anti-infective recommendations included in the treatment protocol, regular training courses for pediatric hematologists and increasing experience may be the reason for reduced infection-related mortality in children with AML. Further studies are needed to decrease infection-related morbidity.
Infektionen sind eine wichtige Ursache für die Morbidität und Mortalität während der Behandlung von Kindern mit akuter myeloischer Leukämie (AML). In den letzten 20 Jahren konnte die Lebenserwartung dieser Patientengruppe vor allem durch die Intensivierung der Chemotherapie deutlich verbessert werden, wobei durch diese Intensivierung das Risiko für Infektionen zugenommen hat. In der Literatur existieren wenige Daten zur Inzidenz und Charakteristik der infektiö- sen Komplikationen unter Chemotherapie für AML. Um die Supportivtherapie jedoch weiter verbessern zu können bedarf es genauer Kenntnis der Häufigkeit, Erreger und Risikofaktoren während der Therapie einer AML. Die vorliegende Arbeit soll Grundlagen für eine Verbesserung von Prophylaxe und Therapie von infektiösen Komplikationen und möglicherweise auch für die Verbesserung der Gesamtüberlebensrate schaffen. Mit diesem Ziel wurden die Daten von 304 Kindern, die in 30 pädiatrisch- onkologischen Zentren nach dem Protokoll AML-BFM 93 behandelt wurden, ana- lysiert. Insgesamt traten bei den 304 Kindern 855 infektiöse Komplikationen auf, die in Fieber unbekannter Ursache (n=530; 62 %), mikrobiologisch (n=270; 31,6 %) und klinisch dokumentierte Infektionen (n=55; 6,4 %) eingeteilt wurden. 34,5 % der Infektionen traten im Rahmen der Induktionstherapie auf, in der insgesamt 92,1 % der Kinder zumindest eine infektiöse Komplikation erlitten, In 241 Fällen konnten pathogene Keime in der Blutkultur nachgewiesen wer- den: 199 mal handelte es sich um gram-positive (82,6 %) und 42 mal um gram-negative Keime (17,4 %). Unter den gram-postiven Keimen stellte die Gruppe der Staphylokokken (92), insbesondere die der koagulase-negativen Staphylokokken (77), die häufigsten Isolate, gefolgt von Streptokokken (78), darunter vor allem die Gruppe der Vi- ridans-Streptokokken (54). Signifikant häufiger traten Viridans-Streptokokken nach Therapieeinheiten auf, die hochdosiertes Ara-C enthielten. Während 114 infektiöser Komplikationen (13,3 %) wurden radiologisch Zeichen einer Pneumonie festgestellt. Eine Neutropenie als wichtigster Einzelrisikofaktor für Infektionen lag bei 682 (79,8 %) der infektiösen Komplikationen vor. Dabei wurden 85,5 % der nach- gewiesenen Erreger in einer Phase von Neutropenie isoliert. Insgesamt starben 20 Kinder an den Folgen infektiöser Komplikationen, darun- ter fünf von 28 Kindern mit Down-Syndrom (17,9 %). Die meisten infektassozi- ierten Todesfälle ereigneten sich während der frühen Induktionstherapie (11). Zur Verbesserung der Überlebensrate der Kinder, die sich einer Chemotherapie aufgrund von AML unterziehen müssen empfiehlt sich die Behandlung in dafür spezialisierten pädiatrisch-onkologischen Zentren. Außerdem sollte die Schwelle zur stationären Wiederaufnahme der Kinder, insbesondere bei pulmonaler Sym- ptomatik sehr niedrig sein. Zusätzlich ist eine frühzeitige Therapie mit Glyko- peptiden bei Kindern mit Fieber und Neutropenie insbesondere nach Therapie- blöcken, die hoch dosiertes Ara-C enthielten, zu erwägen.
Psoriasis is a frequent and often severe inflammatory skin disease, characterized by altered epidermal homeostasis. Since we found previously that Akt/mTOR signaling is hyperactivated in psoriatic skin, we aimed at elucidating the role of aberrant mTORC1 signaling in this disease. We found that under healthy conditions mTOR signaling was shut off when keratinocytes switch from proliferation to terminal differentiation. Inflammatory cytokines (IL-1β, IL-17A, TNF-α) induced aberrant mTOR activity which led to enhanced proliferation and reduced expression of differentiation markers. Conversely, regular differentiation could be restored if mTORC1 signaling was blocked. In mice, activation of mTOR through the agonist MHY1485 also led to aberrant epidermal organization and involucrin distribution. In summary, these results not only identify mTORC1 as an important signal integrator pivotal for the cells fate to either proliferate or differentiate, but emphasize the role of inflammation-dependent mTOR activation as a psoriatic pathomechanism.
The myocyte enhancer factor 2 (MEF2) regulates transcription in cardiac myocytes and adverse remodeling of adult hearts. Activators of G protein‐coupled receptors (GPCRs) have been reported to activate MEF2, but a comprehensive analysis of GPCR activators that regulate MEF2 has to our knowledge not been performed. Here, we tested several GPCR agonists regarding their ability to activate a MEF2 reporter in neonatal rat ventricular myocytes. The inflammatory mediator prostaglandin E2 (PGE2) strongly activated MEF2. Using pharmacological and protein‐based inhibitors, we demonstrated that PGE2 regulates MEF2 via the EP3 receptor, the βγ subunit of Gi/o protein and two concomitantly activated downstream pathways. The first consists of Tiam1, Rac1, and its effector p21‐activated kinase 2, the second of protein kinase D. Both pathways converge on and inactivate histone deacetylase 5 (HDAC5) and thereby de‐repress MEF2. In vivo, endotoxemia in MEF2‐reporter mice induced upregulation of PGE2 and MEF2 activation. Our findings provide an unexpected new link between inflammation and cardiac remodeling by de‐repression of MEF2 through HDAC5 inactivation, which has potential implications for new strategies to treat inflammatory cardiomyopathies.
Seit Einführung der Operationen mit HLM vor etwa 50 Jahren haben zahlreiche Veränderungen von Komponenten zu Fortschritten bezüglich einer verbesserten Biokompatibilität der EKZ geführt (Daniel S 1996; Gu Y 1993). Die EKZ ist nach wie vor eine Voraussetzung für die Mehrzahl der Herzoperationen und ist mit einer systemischen inflammatorischen Reaktion verbunden (SIRS), welche für eine postoperative Organdysfunktion verantwortlich sein kann. Herz-, Lungen- und Nierenfunktion als auch Gerinnungssystem sind in verschiedenem Ausmaß betroffen (Kirklin JK (1983); Westaby S 1987). Dieser Ganzkörperschaden ist ein multifaktorieller Prozess, der hauptsächlich durch den Kontakt von Blut mit fremden Oberflächen verursacht wird und sowohl zu einer Aktivierung der Leukozyten als auch der Komplementkaskade führt (Gourlay T 2001; Levy JH 2003). In Zeiten der minimal-invasiven Herzchirurgie besteht nun nicht nur die Absicht, das chirurgische Trauma zu reduzieren, sondern auch die pathologischen Auswirkungen der EKZ zu minimieren. Deshalb führten neuartige Konzepte und Strategien zur Entwicklung minimierter extrakorporaler Zirkulationssysteme, die vor allem durch reduzierte Fremdoberflächen und geringeres Primingvolumen charakterisiert sind (von Segesser LK 2003). Das CorX™ System folgt diesem Konzept und besteht aus einem geschlossenen System mit sehr geringere Oberfläche, einer integrierten Pumpe mit Oxygenator und einem Luftelimierungssystem. Außerdem wird weder ein Kardiotomie-Reservoir noch eine perikardiale Saugung verwendet, um den zusätzlichen Blutluftkontakt zu vermeiden. Das Ziel dieser Untersuchung war, dass CorX™ System in einer prospektiv randomisierten Studie mit einer konventionellen HLM in Bezug auf perioperative klinische und biochemische Parameter bei koronararteriellen Bypassoperationen in einem Niederigrisiko Patientenkollektiv zu vergleichen. 200 Patienten wurden entweder der CorX™ System-Gruppe (n = 100, Gruppe A) oder der Standard-HLM-Gruppe (n = 100, Gruppe B) zugeteilt. Neben der Auswertung von perioperativen Daten und routinemäßigen Blutproben waren vor allem die Lungenfunktion und der Myokardschaden Gegenstand dieser Untersuchung. Die PMNElastase und TCC dienten dazu, die inflammatorische Reaktion zu beurteilen. Demographische und operative Patientendaten unterschieden sich zwischen beiden Gruppen nicht, sie waren daher vergleichbar. Die postoperative Lungenfunktion zeigte postoperativ keinen signifikanten Unterschied in beiden Gruppen. Zwar war der Oxygenierungsindex in der ersten postoperativen Messung in der CorX-Gruppe signifikant niedriger, dies hatte aber keine Auswirkungen auf die Ergebnisse der Lungenfunktionstests am 5. postoperativen Tag. In der CorX™-Gruppe beobachteten wir verminderte CK-MB Werte postoperativ, was auf einen signifikant niedrigeren Herzmuskelzelluntergang hinweist. Auch dieses Ergebnis hatte aber keine klinische Relevanz. Eine Stunde nach HLM waren sowohl PMN-Elastase als auch TCC signifikant niedriger in Gruppe A im Vergleich zu Gruppe B (PMNE:76±44ng/ml vs 438±230ng/ml, p<0,0001; TCC: 16±8IU/ml vs 29±19IU/ml, p<0,0001). Dies zeigt eine deutlich verminderte inflammatorische Reaktion in der CorX-Gruppe auf, welche sich aber klinisch in diesem Niedrigrisikopatientengut nicht ausgewirkt hat. Die klinischen Ergebnisse hinsichtlich Morbidität und Mortalität, Intensiv- und Krankenhausaufenthalt waren nicht signifikant unterschiedlich im Gruppenvergleich. Das CorX™ System ist sicher und erfordert nur eine kurze Trainingsperiode für das chirurgische Team. Es wurde von den Kardiotechnikern gut angenommen und sogar gegenüber der konventionellen HLM favorisiert. Die inflammatorische Reaktion nach EKZ war signifikant reduziert, jedoch gab es bei Niedrigrisikopatienten keinen messbaren Unterschied bei Betrachtung der klinischen Ergebnisse. Die Reduktion der inflammatorischen Reaktion durch den Einsatz einer minimierten HLM könnte bei Hochrisikopatienten aber von großer Bedeutung sein.
The widely varying therapeutic response of patients with inflammatory bowel disease (IBD) continues to raise questions regarding the unclarified heterogeneity of pathological mechanisms promoting disease progression. While biomarkers for the differentiation of Crohn’s disease (CD) versus ulcerative colitis (UC) have been suggested, specific markers for a CD subclassification in ileal CD versus colonic CD are still rare. Since an altered signature of the tryptophan metabolism is associated with chronic inflammatory disease, we sought to characterize potential biomarkers by focusing on the downstream enzymes and metabolites of kynurenine metabolism. Using immunohistochemical stainings, we analyzed and compared the mucosal tryptophan immune metabolism in bioptic samples from patients with active inflammation due to UC or CD versus healthy controls. Localization-specific quantification of immune cell infiltration, tryptophan-metabolizing enzyme expression and mucosal tryptophan downstream metabolite levels was performed. We found generally increased immune cell infiltrates in the tissue of all patients with IBD. However, in patients with CD, significant differences were found between regulatory T cell and neutrophil granulocyte infiltration in the ileum compared with the colon. Furthermore, we observed decreased kynurenine levels as well as strong kynureninase (KYNU) expression specifically in patients with ileal CD. Correspondingly, significantly elevated levels of the kynurenine metabolite 3-hydroxyanthranilic acid were detected in the ileal CD samples. Highlighting the heterogeneity of the different phenotypes of CD, we identified KYNU as a potential mucosal biomarker allowing the localization-specific differentiation of ileal CD versus colonic CD.
