On 26th November 2010 around 3000 psychiatrists rose up for a minute's silence in the great hall of the International Congress Centrum in Berlin. In der vorliegenden Studie wurden insgesamt 70 Wurzelkanalmodelle, in sieben Gruppen unterteilt, mit einem Handinstrument und 3 verschiedenen maschinell betriebenen Instrumentenprototypen aus einer Nickel-Titanlegierung aufbereitet. Die konventionelle Handaufbereitung durch die Ergoflex-Stahlfeile war durch einen starken Kanalwandabtrag an der Innenkurvatur (straightening) besonders im Mitteldrittel (Messpunkt 3 bis 5) und im koronalen Anteil sowohl innen als auch außen gekennzeichnet. Die maschinelle Aufbereitung durch die drei Prototypen a1, a2 und b zeigte, dass die Aufbereitung mit rotierenden Nickel-Titan-Instrumenten insgesamt etwas gleichmäßiger erfolgt. Aber an Messpunkt 1-3 (1-4 mm vom Apex) im apikalen Drittel des Wurzelkanals an der Außenkurvatur führen sie zu mehr Materialabtrag als die Ergoflex-Stahlfeile. Ab Messpunkt 4 (7mm vom Apex) wird durch die Prototypen mehr an der Innenkurvatur abgetragen. Bei Prototyp b schien das andere Design (Öffnungs-, Tangenten- und Spiralwinkel) mit seinen abgeflachten Schneiden („radial lands“) eine bessere Wurzelkanalzentrierung mit weniger Materialabtrag und weniger Frakturen zu bewirken. Bei der Untersuchung der Aufbereitungszeit waren die maschinell betriebenen Prototypen der Handaufbereitung überlegen. Ob die längeren Aufbereitungszeiten bei Anwendung von Prototyp a1 und a2 im Vergleich zu Prototyp b durch die vorsichtigere Handhabung des Anwenders aufgrund der vielen aufgetretenen Instrumentenfrakturen entstanden, müsste eine weitere Untersuchung aufklären. Somit ist die Aufbereitungszeit kritisch zu hinterfragen. Negativ fiel die hohe Anzahl von Frakturen bei Verwendung der Prototypen a1 und a2 auf. Die Anwendung dieser hauptsächlich im Bereich des apikalen Drittels frakturierten Instrumente am Patienten ist aus diesem Grund zu überdenken. Bei Betrachtung der untersuchten maschinellen Prototypen 1C und 2W (TCM Endo) muß man aufgrund der starken Überschreitung der eingestellten Grenzdrehmomentwerte, der schlechten Beibehaltung der Umdrehungszahlen und schlechteren Taktilität durch den schlechten Sitz der Untersetzungswinkelstücke noch weitere Verbesserungen auf diesen Gebieten fordern, um den Anwender bei der maschinellen Aufbereitung unterstützen zu können.
Background: The management of intraductal papilloma without atypia (IDP) in breast needle biopsy remains controversial. This study investigates the upgrade rate of IDP to carcinoma and clinical and radiologic features predictive of an upgrade. Methods: Patients with a diagnosis of IDP on image-guided (mammography, ultrasound, magnetic resonance imaging) core needle or vacuum-assisted biopsy and surgical excision of this lesion at a certified breast center between 2007 and 2017 were included in this institutional review board-approved retrospective study. Appropriate statistical tests were performed to assess clinical and radiologic characteristics associated with an upgrade to malignancy at excision. Results: For 60 women with 62 surgically removed IDPs, the upgrade rate to malignancy was 16.1% (10 upgrades, 4 invasive ductal carcinoma, 6 ductal carcinoma in situ). IDPs with upgrade to carcinoma showed a significantly greater distance to the nipple (63.5 vs. 36.8 mm; p = 0.012). No significant associations were found between upgrade to carcinoma and age, menopausal status, lesion size, microcalcifications, BI-RADS descriptors, initial BI-RADS category, and biopsy modality. Conclusion: The upgrade rate at excision for IDPs diagnosed with needle biopsy was higher than expected according to some guideline recommendations. Observation only might not be appropriate for all patients with IDP, particularly for those with peripheral IDP.
Background: To evaluate the diagnostic performance of radiomic signatures extracted from contrast-enhanced magnetic resonance imaging (CE-MRI) for the assessment of breast cancer receptor status and molecular subtypes.
Methods: One hundred and forty-three patients with biopsy-proven breast cancer who underwent CE-MRI at 3 T were included in this IRB-approved HIPAA-compliant retrospective study. The training dataset comprised 91 patients (luminal A, n = 49; luminal B, n = 8; HER2-enriched, n = 11; triple negative, n = 23), while the validation dataset comprised 52 patients from a second institution (luminal A, n = 17; luminal B, n = 17; triple negative, n = 18). Radiomic analysis of manually segmented tumors included calculation of features derived from the first-order histogram (HIS), co-occurrence matrix (COM), run-length matrix (RLM), absolute gradient (GRA), autoregressive model (ARM), discrete Haar wavelet transform (WAV), and lesion geometry (GEO). Fisher, probability of error and average correlation (POE + ACC), and mutual information coefficients were used for feature selection. Linear discriminant analysis followed by k-nearest neighbor classification (with leave-one-out cross-validation) was used for pairwise radiomic-based separation of receptor status and molecular subtypes. Histopathology served as the standard of reference.
Results: In the training dataset, radiomic signatures yielded the following accuracies > 80%: luminal B vs. luminal A, 84.2% (mainly based on COM features); luminal B vs. triple negative, 83.9% (mainly based on GEO features); luminal B vs. all others, 89% (mainly based on COM features); and HER2-enriched vs. all others, 81.3% (mainly based on COM features). Radiomic signatures were successfully validated in the separate validation dataset for luminal A vs. luminal B (79.4%) and luminal B vs. triple negative (77.1%).
Conclusions: In this preliminary study, radiomic signatures with CE-MRI enable the assessment of breast cancer receptor status and molecular subtypes with high diagnostic accuracy. These results need to be confirmed in future larger studies.
Background: The angiogenic function of endothelial cells is regulated by numerous mechanisms, but the impact of long noncoding RNAs (lncRNAs) has hardly been studied. We set out to identify novel and functionally important endothelial lncRNAs.
Methods: Epigenetically controlled lncRNAs in human umbilical vein endothelial cells were searched by exon-array analysis after knockdown of the histone demethylase JARID1B. Molecular mechanisms were investigated by RNA pulldown and immunoprecipitation, mass spectrometry, microarray, several knockdown approaches, CRISPR-Cas9, assay for transposase-accessible chromatin sequencing, and chromatin immunoprecipitation in human umbilical vein endothelial cells. Patient samples from lung and tumors were studied for MANTIS expression.
Results: A search for epigenetically controlled endothelial lncRNAs yielded lncRNA n342419, here termed MANTIS, as the most strongly regulated lncRNA. Controlled by the histone demethylase JARID1B, MANTIS was downregulated in patients with idiopathic pulmonary arterial hypertension and in rats treated with monocrotaline, whereas it was upregulated in carotid arteries of Macaca fascicularis subjected to atherosclerosis regression diet, and in endothelial cells isolated from human glioblastoma patients. CRISPR/Cas9-mediated deletion or silencing of MANTIS with small interfering RNAs or GapmeRs inhibited angiogenic sprouting and alignment of endothelial cells in response to shear stress. Mechanistically, the nuclear-localized MANTIS lncRNA interacted with BRG1, the catalytic subunit of the switch/sucrose nonfermentable chromatin-remodeling complex. This interaction was required for nucleosome remodeling by keeping the ATPase function of BRG1 active. Thereby, the transcription of key endothelial genes such as SOX18, SMAD6, and COUP-TFII was regulated by ensuring efficient RNA polymerase II machinery binding.
Conclusion: MANTIS is a differentially regulated novel lncRNA facilitating endothelial angiogenic function.
Compared to their protein-coding counterparts, almost nothing is known about the role of long noncoding RNAs (lncRNAs) in cardiac fibrosis. In the current report, Liang and Pan et al. characterized the pro-fibrotic lncRNA PFL in respect to cardiac fibrosis in mice. PFL was upregulated in the hearts of mice after myocardial infarction and in fibrotic cardiac fibroblasts. Moreover, PFL competitively sponged the cardio-protective miRNA let-7d in cardiac fibroblasts. Knockdown of platelet activating factor receptor (PTAFR) was shown to affect the pro-fibrotic collagen production mediated by PFL. PTAFR overexpression also led to collagen production and RNA abundance of PTAFR was also regulated by miRNA let-7d. Therefore, the PFL/PTAFR/let-7d-dependent gene regulatory mechanism proposed by the authors manifests the hypothesis of competing endogenous RNAs to cardiac fibrosis.
A 79 year old female patient was admitted to our emergency department with a fracture of the right medial femoral neck six days after a fall on her right side and a cemented hemiprosthesis was implanted. Five days later, she developed a hemorrhagic shock and was diagnosed with a delayed splenic rupture and the spleen was resected. Histopathological examination showed a delayed rupture of an otherwise normal spleen without signs of an underlying pathology. The outcome was fatal: In the postoperative course she developed pneumonia, three weeks later she succumbed due to multiple organ failure.
Even careful reevaluation of the case did not provide any clues to expect an injury of the spleen according to trauma mechanism.
This case shows that delayed splenic rupture of a normal spleen may occur even after a low energy trauma. Injury of the spleen should therefore always be considered, even with an uncharacteristic anamnesis. Physical examination after trauma should therefore always include a careful clinical evaluation. The clinical threshold for a FAST examination should be low.
The coincidence of a femoral neck fracture and a splenic rupture after a low energy trauma has not been reported before.
Objective: Clostridial gas gangrene (GG) or clostridial myonecrosis is a very rare but life‐threatening necrotizing soft tissue infection (NSTI) caused by anaerobic, spore‐forming, and gas‐producing clostridium subspecies. It is the most rapidly spreading and lethal infection in humans, also affecting muscle tissue. The high mortality, of up to 100%, in clostridial GG is mediated by potent bacterial exotoxins. Necrotizing fasciitis (NF) is an important differential diagnosis, most often caused by group A streptococci, primarily not affecting musculature but the subcutaneous tissue and fascia. In the early stages of the infection, it is difficult to distinguish between GG and NF. Therefore, we compare both infection types, identify relevant differences in initial clinical presentation and later course, and present the results of our patients in a retrospective review.
Methods: Patients diagnosed with GG from 2008 to 2018 in our level one trauma center were identified. Their charts were reviewed retrospectively and data analyzed in terms of demographic information, microbiological and histological results, therapeutic course, outcome, and mortality rates. The laboratory risk indicator for NF (LRINEC) score was applied on the first blood work acquired. Results were compared to those of a second group diagnosed with NF.
Results: Five patients with GG and nine patients with NF were included in the present study. Patients with GG had a mortality rate of 80% compared to 0% in patients with NF. In eight patients with NF, affected limbs could be salvaged; one NF underwent amputation. LRINEC did not show significant differences between the groups; however, C‐reactive protein was significantly increased (P = 0.009) and hemoglobin (Hb) was significantly decreased (P = 0.02) in patients with GG. Interleukin‐6 and procalcitonin levels did not show significant difference. Patients with GG were older (70.2 vs 50 years). Of the isolated bacteria, 86% were sensitive to the initial calculated antibiotic treatment with ampicillin‐sulbactam or imipenem plus metronidazole plus clindamycin.
Conclusion: Both GG and NF need full‐scale surgical, antibiotic, and intensive care treatment, especially within the first days. Among patients with NSTI, those with clostridial GG have a significantly increased mortality risk due to early septic shock caused by clostridial toxins. In the initial stages, clinical differences are hardly detectable. Immediate surgical debridement is the key to successful therapy for NSTI and needs to be performed as early as possible. However, patients should be treated in a center with an experienced interdisciplinary intensive care team based on a predetermined treatment plan.
Purpose: Fractures of the humerus account for 5%–8% of all fractures. Nonunion is found with an incidence of up to 15%, depending on the location of the fracture. In case of a manifest nonunion the surgeon faces a challenging problem and has to conceive a therapy based on the underlying pathology. The aim of this study was to describe our treatment concepts for this entity and present our results of the last five years.
Methods: Twenty-six patients were treated for nonunion of the humerus between January 2013 and December 2017. Their charts were reviewed retrospectively and demographic data, pathology, surgical treatment and outcome were assessed.
Results: The most frequent location for a nonunion was the humeral shaft, with the most common trauma mechanism being multiple falls. Most often atrophic nonunion (n = 14), followed by hypertrophic and infection-caused nonunion (each n = 4), were found. Our treatment concept could be applied in 19 patients, of which in 90% of those who were available for follow-up consolidation could be achieved.
Conclusion: Humeral nonunion is a heterogeneous entity that has to be analyzed precisely and be treated correspondingly. We therefore present a treatment concept based on the underlying pathology.
The Masquelet technique for the treatment of large bone defects is a two-stage procedure based on an induced membrane. We eliminate the first surgical step by using a decellularized dermal skin graft (Epiflex®) populated with bone marrow mononuclear cells (BMC), as a replacement for the induced membrane. The aim of this study was to demonstrate the feasibility of this technology and provide evidence of equivalent bone healing in comparison to the induced membrane-technique. Therefore, 112 male Sprague–Dawley rats were allocated in six groups and received a 10 mm femoral defect. Defects were treated with either the induced membrane or decellularized dermis, with or without the addition of BMC. Defects were then filled with a scaffold (β-TCP), with or without BMC. After a healing time of eight weeks, femurs were taken for histological, radiological and biomechanical analysis. Defects treated with Epiflex® showed increased mineralization and bone formation predominantly in the transplanted dermis surrounding the defect. No significant decrease of biomechanical properties was found. Vascularization of the defect could be enhanced by addition of BMC. Considering the dramatic reduction of a patient’s burden by the reduced surgical stress and shortened time of treatment, this technique could have a great impact on clinical practice.
The Masquelet technique for the treatment of large bone defects is a two‐stage procedure based on an induced membrane. The size of a scaffold is reported to be a critical factor for bone healing response. We therefore aimed to investigate the influence of the granule size of a bone graft substitute on bone marrow derived mononuclear cells (BMC) supported bone healing in combination with the induced membrane. We compared three different sizes of Herafill® granules (Heraeus Medical GmbH, Wehrheim) with or without BMC in vivo in a rat femoral critical size defect. A 10 mm defect was made in 126 rats and a membrane induced by a PMMA‐spacer. After 3 weeks, the spacer was taken out and membrane filled with different granule sizes. After 8 weeks femurs were taken for radiological, biomechanical, histological, and immunohistochemical analysis. Further, whole blood of the rat was incubated with granules and expression of 29 peptide mediators was assessed. Smallest granules showed significantly improved bone healing compared to larger granules, which however did not lead to an increased biomechanical stability in the defect zone. Small granules lead to an increased accumulation of macrophages in situ which could be assigned to the inflammatory subtype M1 by majority. Increased release of chemotactic respectively proangiogenic active factors in vitro compared to syngenic bone and beta‐TCP was observed. Granule size of the bone graft substitute Herafill® has significant impact on bone healing of a critical size defect in combination with Masquelet's technique in terms of bone formation and inflammatory.
Background: Available data on the incidence and outcome of invasive fungal diseases (IFD) in children with hematological malignancies or after allogeneic hematopoietic stem cell transplantation (HSCT) are mostly based on monocenter, retrospective studies or on studies performed prior to the availability of newer triazoles or echinocandins.
Procedure: We prospectively collected clinical data on incidence, diagnostic procedures, management and outcome of IFD in children treated for hematological malignancies or undergoing HSCT in three major European pediatric cancer centers.
Results: A total of 304 children (median age 6.0 years) who underwent 360 therapies (211 chemotherapy treatments, 138 allogeneic HSCTs and/or 11 investigational chemotherapeutic treatments) were included in the analysis. Nineteen children developed proven/probable IFD, mostly due to Aspergillus (n = 10) and Candida spp. (n = 5), respectively. In patients receiving chemotherapy, 11 IFDs occurred, all during induction or re-induction therapy. None of these patients died due to IFD, whereas IFD was lethal in 3 of the 8 HSCT recipients with IFD. Significant differences among centers were observed with regard to the use of imaging diagnostics and the choice, initiation and duration of antifungal prophylaxis.
Conclusion: This prospective multicenter study provides information on the current incidence and outcome of IFD in the real life setting. Practice variation between the centers may help to ultimately improve antifungal management in children at highest risk for IFDs.
Despite the availability of new antifungal compounds, invasive fungal infection remains a significant cause of morbidity and mortality in children and adults undergoing allogeneic hematopoietic stem cell transplantation (HSCT). Allogeneic HSCT recipients suffer from a long lasting defect of different arms of the immune system, which increases the risk for and deteriorates the prognosis of invasive fungal infections. In turn, advances in understanding these immune deficits have resulted in promising strategies to enhance or restore critical immune functions in allogeneic HSCT recipients. Potential approaches include the administration of granulocytes, since neutropenia is the single most important risk factor for invasive fungal infection, and preliminary clinical results suggest a benefit of adoptively transferred donor-derived antifungal T cells. In vitro data and animal studies demonstrate an antifungal effect of natural killer cells, but clinical data are lacking to date. This review summarizes and critically discusses the available data of immunotherapeutic strategies in allogeneic HSCT recipients suffering from invasive fungal infection.
Invasive mold disease (IMD) of the central nervous system (CNS) is a severe infectious complication in immunocompromised patients, but early microbiological diagnosis is difficult. As data on the value of biomarkers in the CNS are scarce, in particular in children, we retrospectively analyzed the performance of galactomannan (GM) and PCR assays in CNS samples of 15 children with proven and probable CNS IMD and of 32 immunocompromised children without fungal infection. Galactomannan in the cerebrospinal fluid (CSF) was assessed in nine of the 15 pediatric patients and was positive in five of them. Polymerase chain reaction (PCR) was performed in eight of the 15 patients and detected nucleic acids from molds in six patients. Galactomannan and PCR in CNS samples were the only positive microbiologic parameter in the CNS in three and two patients, respectively. In four patients, PCR specified the pathogen detected in microscopy. Galactomannan and PCR results remained negative in the CSF of all immunocompromised children without evidence for CNS IMD. Our data suggest that GM and PCR in CNS specimens are valuable additional tools in diagnosing CNS IMD and should be included in the work up of all pediatric patients with suspected mold disease of the CNS.
Diagnostic approaches for invasive aspergillosis—specific considerations in the pediatric population

Invasive aspergillosis (IA) is a major cause of morbidity and mortality in children with hematological malignancies and those undergoing hematopoietic stem cell transplantation. Similar to immunocompromised adults, clinical signs, and symptoms of IA are unspecific in the pediatric patient population. As early diagnosis and prompt treatment of IA is associated with better outcome, imaging and non-invasive antigen-based such as galactomannan or ß-D-glucan and molecular biomarkers in peripheral blood may facilitate institution and choice of antifungal compounds and guide duration of therapy. In patients in whom imaging studies suggest IA or another mold infection, invasive diagnostics such as bronchoalveolar lavage and/or bioptic procedures should be considered. Here we review the current data of diagnostic approaches for IA in the pediatric setting and highlight the major differences of performance and clinical utility of the tests between children and adults.
Background: Standardization in clinical practice may lead to improved outcomes. Unfortunately, little is known about the variability of non-pharmacological anti-infective measures in children with cancer.
Design and Methods: A web-based survey assessed institutional recommendations regarding restrictions of social contacts, pets and food and instructions on wearing face masks in public for children with standard- risk acute lymphoblastic leuk emia and acute myeloid leukemia during intensive chemotherapy.
Results: A total of 336 institutions in 27 countries responded to the survey (range, 1-76 institutions per country; overall response rate 61%). Most institutions recommend that patients with acute myeloid leukemia avoid indoor public places and daycare, kindergarten and school, whereas recommendations for patients with acute lymphoblastic leukemia differ considerably by institution. In terms of restrictions related to pets, there was a wide variability between institutions for both acute lymphoblastic and acute myeloid leukemia patients. Most, but not all institutions do not allow children with either acute lymphoblastic or acute myeloid leukemia to eat raw meat, raw seafood or unpasteurized milk. Whereas most institutions do not routinely recommend that patients with acute lymphoblastic leukemia wear face masks in public, advice on this matter varies for patients with acute myeloid leukemia.
Conclusions: The survey demonstrates that there is a wide variation in recommendations on non-pharmacological anti-infective measures between different institutions, countries and continents. This information may be used to encourage harmonization of supportive care practices and future clinical trials.
Background: There are no validated standardised clinical procedures for severity measurement of acute bronchitis in children. The "BSS-ped", a short version of the physician-rated assessment scale BSS (Bronchitis Severity Scale), can fill this gap, if it is valid.
Objective: To examine the scale´s validity.
Methods: Investigations were planned according to classical clinical-psychometric validity criteria including a formal competence evaluation of the scale´s authors and statistical analyses of data from 78 patients aged 1-6 and diagnosed with "acute bronchitis". Cross-validation was provided by analysis of data from 70 children with matching age, sex and diagnosis. All children were examined three times (day 0, 3-5 and 7) using the BSS-ped in addition to other clinical and psychometric monitoring procedures.
Results: The evidently high level of expertise of the scale’s authors substantiates pronounced content validity and relevance of the BSS-ped and its items. The validity criterion, i.e. to reflect the unidimensional severity of acute bronchitis and its change using the BSS-ped score, was fulfilled. There were substantial correlations with other scales measuring the current health-related quality of life, as well as satisfaction and success of treatment. Severity change prognoses for acute bronchitis under placebo and an active substance were correct. The BSS-ped was found to be a feasible instrument because it can be repeated at short intervals (minute range) without any special technical aids or extended training.
Conclusion: The BSS-ped is a valid procedure for measuring the severity of acute bronchitis in children.
Background: Reactive oxygen species (ROS) and reactive nitrogen species (RNS) are produced during hemorrhagic shock and resuscitation (H/R), which may contribute to multiple organ failure. The AIM of this study was to test the hypothesis that green tea (Camellia sinenesis) extract containing 85% polyphenols decreases injury after H/R in rats by scavenging ROS and RNS. Method: S: Female Sprague Dawley rats were given 100 mg polyphenol extract/kg body weight or vehicle 2 h prior to hemorrhagic shock. H/R was induced by two protocols: 1) withdrawal of blood to a mean arterial pressure of 40 mm Hg followed by further withdrawals to decrease blood pressure progressively to 28 mm Hg over 1 h (severe), and 2) withdrawal of blood to a sustained hypotension of 40 mm Hg for 1 h (moderate). Rats were then resuscitated over 1 h with 60% of the shed blood volume plus twice the shed blood volume of lactated Ringer's solution. Serum samples were collected at 10 min and 2 h after resuscitation. At 2 or 18 h, livers were harvested for cytokine and 3-nitrotyrosine quantification, immunohistochemical detection of 4-hydroxynonenol (4-HNE) and inducible nitric oxide synthase (iNOS) protein expression. Results: After severe H/R, 18-h survival increased from 20% after vehicle to 70% after polyphenols (p<0.05). After moderate H/R, survival was greater (80%) and not different between vehicle and polyphenols. In moderate H/R, serum alanine aminotransferase (ALT) increased at 10 min and 2 h postresuscitation to 345 and 545 IU/L, respectively. Polyphenol treatment blunted this increase to 153 and 252 IU/L at 10 min and 2 h (p<0.01). Polyphenols also blunted increases in liver homogenates of TNFalpha (7.0 pg/mg with vehicle vs. 4.9 pg/mg with polyphenols, p<0.05), IL-1beta (0.80 vs. 0.37 pg/mg, p<0.05), IL-6 (6.9 vs. 5.1 pg/mg, p<0.05) and nitrotyrosine (1.9 pg/mg vs. 0.6 pg/mg, p<0.05) measured 18 h after H/R. Hepatic 4-HNE immunostaining indicative of lipid peroxidation also decreased from 4.8% after vehicle to 1.5% after polyphenols (p<0.05). By contrast, polyphenols did not block increased iNOS expression at 2 h after H/R. CONCLUSION: Polyphenols decrease ROS/RNS formation and are beneficial after hemorrhagic shock and resuscitation.
Das idiopathische Parkinsonsyndrom (IPS) wurde durch James Parkinson im Jahr 18171 vornehmlich als neurologische Bewegungsstörung beschrieben. Durch die Verdienste umfassender Forschung und klinischer Diagnostik der letzten Jahrzehnte wird das IPS heute als Syndrom aufgefasst, das neben motorischen auch durch neuropsychiatrische, vegetative und sensible Symptome charakterisiert ist. Eines dieser sogenannten „nicht-motorischen Symptome“ bildet die (milde) kognitive Beeinträchtigung, die jeden vierten nicht-dementen Parkinsonpatienten betrifft2 und die im oft langjährigen Krankheitsverlauf in eine Parkinsondemenz münden kann, an der 8 Jahre nach Krankheitsbeginn bis zu 78% der Parkinsonpatienten leiden.3 Durch die steigende Lebenserwartung der Patienten und die besseren Therapieoptionen der motorischen Komponente eines IPS finden derzeit gerade die nicht-motorischen Symptome sowohl in der Forschung als auch der Klinik zunehmend stärkere Beachtung.
Die motorische Symptomatik des IPS wird vorrangig durch die Degeneration dopaminerger Neuronen in bestimmten Strukturen des Hirnstamms hervorgerufen.4 Die Entwicklung kognitiver Symptome ist komplexer und umfasst vermutlich neben der (genannten) dopaminergen Degeneration die Beeinträchtigung weiterer Neurotransmitter sowie degenerative Prozesse an anderer Stelle des ZNS.5 Insbesondere die Atrophie von frontalem, parietalem und (medio-) temporalem Cortex scheint mit der Parkinsondemenz assoziiert.6–8 Auch gleichzeitig auftretende histopathologische Prozesse, die für die Alzheimer-Demenz charakteristisch sind, werden diskutiert.9
Der Hippocampus, eine Struktur des Mediotemporallappens, leistet einen bedeutenden Beitrag zum deklarativen räumlichen Gedächtnis und zum Arbeitsgedächtnis sowie zur Verarbeitung von Emotionen, sodass er beim normalen Altern, aber auch vielen Erkrankungen, eine wichtige Rolle einnimmt.10–12 Vor allem bei der Alzheimer-Demenz ist der Hippocampus eine sehr früh von der Atrophie betroffene Struktur.13 In den letzten Jahren mehren sich die Hinweise, dass dies auch bei der Parkinsondemenz der Fall ist.14–16 Aus diesem Grund wird im Folgenden ein besonderes Augenmerk auf diese Struktur gelegt.
Die vorliegende Studie setzt sich mit der Fragestellung auseinander, ob und inwiefern sich Parkinsonpatienten mit unterschiedlich ausgeprägten kognitiven Einschränkungen in den Volumina ausgewählter Hirnstrukturen – darunter die corticale graue und weiße 8 Substanz, der Hippocampus und die Hirnventrikel – sowie in den neuropsychologischen Domänen Gedächtnis, Exekutivfunktionen und Aufmerksamkeit, Sprache und visuell-räumliche Funktionen unterscheiden. Außerdem wird untersucht, ob zwischen den Hirnvolumina und dem Grad der kognitiven Einschränkung ein Zusammenhang besteht. Hierzu werden Parkinsonpatienten ohne Demenz (PD), Parkinsonpatienten mit milder kognitiver Beeinträchtigung (PD-MCI) und Patienten mit Parkinsondemenz (PDD) neuropsychologisch untersucht und magnetresonanztomographische Aufnahmen des Gehirns erstellt. Die mit einem automatisierten Messprogramm ermittelten Hirnvolumina werden in Korrelation zu Testungen des Gedächtnisses, der Exekutive und Aufmerksamkeit, der Sprache und visuell-räumlicher Funktionen gesetzt.
Die Arbeit gliedert sich wie folgt: Zum besseren Verständnis der kognitiven Symptomatik werden im ersten Kapitel die nicht-kognitiven Symptome des IPS charakterisiert sowie der aktuelle Wissensstand über Kognition beim IPS wiedergegeben. Anschließend werden die technischen Grundlagen der Magnetresonanztomographie und Methoden zur Auswertung struktureller MRT-Bilder erläutert. Darauf aufbauend wird die Fragestellung konkretisiert, bevor im zweiten Teil der Arbeit die Vorstellung des Studiendesigns, die Präsentation der Ergebnisse und im letzten Teil die Diskussion erfolgen.
Background: Computed tomography (CT) allows estimation of coronary artery calcium (CAC) progression. We evaluated several progression algorithms in our unselected, population-based cohort for risk prediction of coronary and cardiovascular events.
Methods: In 3281 participants (45–74 years of age), free from cardiovascular disease until the second visit, risk factors, and CTs at baseline (b) and after a mean of 5.1 years (5y) were measured. Hard coronary and cardiovascular events, and total cardiovascular events including revascularization, as well, were recorded during a follow-up time of 7.8±2.2 years after the second CT. The added predictive value of 10 CAC progression algorithms on top of risk factors including baseline CAC was evaluated by using survival analysis, C-statistics, net reclassification improvement, and integrated discrimination index. A subgroup analysis of risk in CAC categories was performed.
Results: We observed 85 (2.6%) hard coronary, 161 (4.9%) hard cardiovascular, and 241 (7.3%) total cardiovascular events. Absolute CAC progression was higher with versus without subsequent coronary events (median, 115 [Q1–Q3, 23–360] versus 8 [0–83], P<0.0001; similar for hard/total cardiovascular events). Some progression algorithms added to the predictive value of baseline CT and risk assessment in terms of C-statistic or integrated discrimination index, especially for total cardiovascular events. However, CAC progression did not improve models including CAC5y and 5-year risk factors. An excellent prognosis was found for 921 participants with double-zero CACb=CAC5y=0 (10-year coronary and hard/total cardiovascular risk: 1.4%, 2.0%, and 2.8%), which was for participants with incident CAC 1.8%, 3.8%, and 6.6%, respectively. When CACb progressed from 1 to 399 to CAC5y≥400, coronary and total cardiovascular risk were nearly 2-fold in comparison with subjects who remained below CAC5y=400. Participants with CACb≥400 had high rates of hard coronary and hard/total cardiovascular events (10-year risk: 12.0%, 13.5%, and 30.9%, respectively).
Conclusions: CAC progression is associated with coronary and cardiovascular event rates, but adds only weakly to risk prediction. What counts is the most recent CAC value and risk factor assessment. Therefore, a repeat scan >5 years after the first scan may be of additional value, except when a double-zero CT scan is present or when the subjects are already at high risk.
Selfish genetic elements that act as post-segregation distorters cause lethality in non-carrier individuals after fertilization. Two post-segregation distorters have been previously identified in Caenorhabditis elegans, the peel-1/zeel-1 and the sup-35/pha-1 elements. These elements seem to act as modification-rescue systems, also called toxin/antidote pairs. Here we show that the maternal-effect toxin/zygotic antidote pair sup-35/pha-1 is required for proper expression of apical junction (AJ) components in epithelia and that sup-35 toxicity increases when pathways that establish and maintain basal epithelial characteristics, die-1, elt-1, lin-26, and vab-10, are compromised. We demonstrate that pha-1(e2123) embryos, which lack the antidote, are defective in epidermal morphogenesis and frequently fail to elongate. Moreover, seam cells are frequently misshaped and mispositioned and cell bond tension is reduced in pha-1(e2123) embryos, suggesting altered tissue material properties in the epidermis. Several aspects of this phenotype can also be induced in wild-type embryos by exerting mechanical stress through uniaxial loading. Seam cell shape, tissue mechanics, and elongation can be restored in pha-1(e2123) embryos if expression of the AJ molecule DLG-1/Discs large is reduced. Thus, our experiments suggest that maternal-effect toxicity disrupts proper development of the epidermis which involves distinct transcriptional regulators and AJ components.
Background: Health Authorities recommend influenza vaccination of healthcare personnel (HCP) to decrease the transmission of influenza to vulnerable patients. Recent studies have almost exclusively used quantitative questionnaires in order to identify determinants of vaccination behaviour. Interviews enable HCP to express freely why they think they are (not) willing to get vaccinated against influenza.
Methods: By means of semi-structured one-on-one interviews with 123 Belgian, Dutch and German HCP, reasons for and against vaccination, experiences with influenza vaccination, intention to get vaccinated and possible barriers, as well as willingness to advice influenza vaccination to patients were investigated. Data were processed with QSR NVivo 8.0 and analysed using a combination of a deductive and a general inductive approach.
Results: Across countries, self-protection, patient protection, and protection of family members were reported as most important reasons to get vaccinated against influenza. Reasons to not get vaccinated against influenza were fear of side effects caused by the vaccine, a low risk-perception, the disbelief in the effectiveness of influenza vaccination, organizational barriers, misconceptions, and undefined negative emotions.
Conclusions: The social cognitive variables underlying the decision of HCP to get vaccinated against influenza (or not) seem to be similar in Belgium, Germany, and the Netherlands, even though some differences surfaced. A quantitative investigation of those social cognitive variables is needed in order to determine the importance of the social cognitive variables in explaining the intention to get vaccinated and the importance of the similarities and differences between countries that have been found in this study.
Background: Influenza vaccination is recommended for all healthcare personnel (HCP) and most institutions offer vaccination for free and on site. However, medical students do not always have such easy access, and the predictors that might guide the motivation of medical students to get vaccinated are largely unknown.
Methods: We conducted a cross-sectional survey study among pre-clinical medical students in a German University hospital to assess the social cognitive predictors of influenza vaccination, as well as reasons for refusal and acceptance of the vaccine.
Results: Findings show that pre-clinical medical students have comparable knowledge gaps and negative attitudes towards influenza vaccination that have previously been reported among HCP. Lower injunctive norms and higher feelings of autonomy contribute to no intention to get vaccinated against influenza, while a positive instrumental attitude and higher feelings of autonomy contribute to a high intention to get vaccinated. The variables in the regression model explained 20% of the variance in intention to get vaccinated. Conclusions: The identified factors should be addressed early in medical education, and hospitals might benefit from a more inclusive vaccination program and accessibility of free vaccines for their medical students.
Vorhofflimmern ist die in der Klinik und Praxis am häufigsten diagnostizierte Herzrhythmusstörung. In allen Fällen bestehen zwei grundsätzliche Therapieoptionen: Zum einen Kardioversionsbehandlung gefolgt von Rezidivprophylaxe mit Antiarrhythmika bzw. alleinige Frequenzkontrolle unter adäquater Antikoagulation bei Inkaufnahme der Rhythmusstörung. In der vorliegenden Arbeit sind 32 Patienten mit persistierendem Vorhofflimmern untersucht worden, die entwedermit Cordichin, Sotalol oder Amiodaron behandelt worden sind. Der Schwerpunkt der Untersuchung wurde auf die Veränderung der Hämodynamik nach erfolgreicher Kardioversion in den Sinusrhythmus gelegt. Hier zeigte sich, dass unabhängig von dem zur Rhythmusstabilisierung verwendeten Medikament die erfolgreiche Wiederherstellung von Sinusrhythmus mit einer Verbesserung der Hämodynamik und der Auswurffraktion des Herzens einherging. Somit bestätigen die vorliegenden Untersuchungsbefunde die umfangreiche Literatur zu diesem Thema.
Comprehensive analysis of tumour sub-volumes for radiomic risk modelling in locally advanced HNSCC

Simple Summary: Radiomic risk models are usually based on imaging features, which are extracted from the entire gross tumour volume (GTV entire ). This approach does not explicitly consider the complex biological structure of the tumours. Therefore, in this retrospective study, we investigated the prognostic value of radiomic analyses based on different tumour sub-volumes using computed tomography imaging of patients with locally advanced head and neck squamous cell carcinoma who were treated with primary radio-chemotherapy. The GTV entire was cropped by different margins to define the rim and corresponding core sub-volumes of the tumour. Furthermore, the best performing tumour rim sub-volume was extended into surrounding tissue with different margins. As a result, the models based on the 5 mm tumour rim and on the 3 mm extended rim sub-volume showed an improved performance compared to models based on the corresponding tumour core. This indicates that the consideration of tumour sub-volumes may help to improve radiomic risk models.
Abstract: Imaging features for radiomic analyses are commonly calculated from the entire gross tumour volume (GTVentire). However, tumours are biologically complex and the consideration of different tumour regions in radiomic models may lead to an improved outcome prediction. Therefore, we investigated the prognostic value of radiomic analyses based on different tumour sub-volumes using computed tomography imaging of patients with locally advanced head and neck squamous cell carcinoma. The GTVentire was cropped by different margins to define the rim and the corresponding core sub-volumes of the tumour. Subsequently, the best performing tumour rim sub-volume was extended into surrounding tissue with different margins. Radiomic risk models were developed and validated using a retrospective cohort consisting of 291 patients in one of the six Partner Sites of the German Cancer Consortium Radiation Oncology Group treated between 2005 and 2013. The validation concordance index (C-index) averaged over all applied learning algorithms and feature selection methods using the GTVentire achieved a moderate prognostic performance for loco-regional tumour control (C-index: 0.61 ± 0.04 (mean ± std)). The models based on the 5 mm tumour rim and on the 3 mm extended rim sub-volume showed higher median performances (C-index: 0.65 ± 0.02 and 0.64 ± 0.05, respectively), while models based on the corresponding tumour core volumes performed less (C-index: 0.59 ± 0.01). The difference in C-index between the 5 mm tumour rim and the corresponding core volume showed a statistical trend (p = 0.10). After additional prospective validation, the consideration of tumour sub-volumes may be a promising way to improve prognostic radiomic risk models.
Cancer stem cells (CSCs) are nowadays one of the major focuses in tumor research since this subpopulation was revealed to be a great obstacle for successful treatment. The identification of CSCs in pediatric solid tumors harbors major challenges because of the immature character of these tumors. Here, we present CD34, CD90, OV-6 and cell-surface vimentin (csVimentin) as reliable markers to identify CSCs in hepatoblastoma cell lines. We were able to identify CSC characteristics for the subset of CD34+CD90+OV-6+csVimentin+-co-expressing cells, such as pluripotency, self-renewal, increased expression of EMT markers and migration. Treatment with Cisplatin as the standard chemotherapeutic drug in hepatoblastoma therapy further revealed the chemo-resistance of this subset, which is a main characteristic of CSCs. When we treated the cells with the Hsp90 inhibitor 17-AAG, we observed a significant reduction in the CSC subset. With our study, we identified CSCs of hepatoblastoma using CD34, CD90, OV-6 and csVimentin. This set of markers could be helpful to estimate the success of novel therapeutic approaches, as resistant CSCs are responsible for tumor relapses.
Background: Serial volumetric changes of reconstructed breasts have not been studied in detail. In this study, we analyzed serial volumetric changes of reconstructed and contralateral normal breasts during long-term follow-up, with a focus on the effect of various adjuvant therapies.
Methods: Among all patients who underwent immediate breast reconstruction with a unilateral pedicled transverse rectus abdominis musculocutaneous (p-TRAM) flap, 42 patients with valid data from ≥3 postoperative positron emission tomography-computed tomography (PET-CT) scans were included. The volumes of the reconstructed and normal breasts were measured, and the ratio of flap volume to that of the contralateral breast was calculated. Serial changes in volume and the volume ratio were described, and the effects of chemotherapy, radiation therapy, and hormone therapy on volumetric changes were analyzed.
Results: The mean interval between the initial reconstruction and each PET-CT scan was 16.5, 30, and 51 months respectively. Thirty-five, 36, and 10 patients received chemotherapy, hormone therapy, and radiation therapy, respectively. The flap volume at each measurement was 531.0, 539.6, and 538.0 cm3, and the contralateral breast volume was 472.8, 486.4, and 500.8 cm3, respectively. The volume ratio decreased from 115.1% to 113.4%, and finally to 109.6% (P=0.02). Adjuvant therapies showed no significant effects.
Conclusions: We demonstrated that the p-TRAM flap maintained its volume over a long-term follow up, while the volume of the contralateral native breast slowly increased. Moreover, adjuvant breast cancer therapies had no statistically significant effects on the volume of the reconstructed p-TRAM flaps or the contralateral native breasts.
PAX2 regulates ADAM10 expression and mediates anchorage-independent cell growth of melanoma cells

PAX transcription factors play an important role during development and carcinogenesis. In this study, we investigated PAX2 protein levels in melanocytes and melanoma cells by Western Blot and immunofluorescence analysis and characterized the role of PAX2 in the pathogenesis of melanoma. In vitro we found weak PAX2 protein expression in keratinocytes and melanocytes. Compared to melanocytes increased PAX2 protein levels were detectable in melanoma cell lines. Interestingly, in tissue sections of melanoma patients nuclear PAX2 expression strongly correlated with nuclear atypia and the degree of prominent nucleoli, indicating an association of PAX2 with a more atypical cellular phenotype. In addition, with chromatin immunoprecipitation assay, PAX2 overexpression and PAX2 siRNA we present compelling evidence that PAX2 can regulate ADAM10 expression, a metalloproteinase known to play important roles in melanoma metastasis. In human tissue samples we found co-expression of PAX2 and ADAM10 in melanocytes of benign nevi and in melanoma cells of patients with malignant melanoma. Importantly, the downregulation of PAX2 by specific siRNA inhibited the anchorage independent cell growth and decreased the migratory and invasive capacity of melanoma cells. Furthermore, the downregulation of PAX2 abrogated the chemoresistance of melanoma cells against cisplatin, indicating that PAX2 expression mediates cell survival and plays important roles during melanoma progression.
Hintergrund und Fragestellung: Die Studie untersucht den Einfluss von Propofol im Vergleich zu Midazolam als Sedativum bei der Ösophagogastroduodenoskopie (ÖGD) im Hinblick auf die psychomotorischen Funktionen von Patienten mit Leberzirrhose und einer minimalen hepatischen Enzephalopathie (HE). Patienten und Methoden: Patienten mit einer Leberzirrhose ohne Zeichen einer klinisch manifesten HE wurden einer ÖGD in therapeutischer Absicht (Ösophagusvarizenligatur) unterzogen. Insgesamt 60 Patienten wurden randomisiert, entweder um Propofol (n = 40) oder Midazolam (n = 20) als Sedativum für die Endoskopie zu erhalten. Die beiden Gruppen waren hinsichtlich ihres Alters, Geschlechts und des Child-Stadiums der Leberzirrhose vergleichbar. Alle Patienten absolvierten zweifach eine psychometrische Testung, den sog. PSE-Syndrom-Test (der u.a. einen Zahlenverbindungstests (ZVT) beinhaltet), einmal vor und 2 Stunden nach Beendigung der Endoskopie. Die Art (diagnostisch/therapeutisch) und Dauer der Endoskopie waren ebenfalls in beiden Gruppen vergleichbar. Die zur Durchführung der Testbatterie benötigten Zeiten als auch der ermittelte PSE-Score wurden dokumentiert. Die Basisdaten vor Endoskopie wurden mit den nach der Endoskopie erzielten Ergebnissen verglichen. Die Aufwachzeit und -qualität nach der Sedierung wurden mit dem sog. Post-Anesthesia Recovery Score (PARS) ermittelt und dokumentiert. Zum Vergleich wurden die psychometrischen Tests auch bei einer „Kontroll-Gruppe“ von 20 lebergesunden Patienten erhoben, welche ebenfalls im Abstand von 2 Stunden evaluiert wurden, ohne sich jedoch einer Endoskopie oder Sedierung zu unterziehen. Ergebnisse: Sowohl die Unterschiede der Zeiten der Durchfürhung des ZVT-A vor und nach Sedierung (mediane delta-Zeit Propofol-Gruppe, -9,5 sec [95 % KI, -15,7 bis -4,6 sec] und Midazolam-Gruppe, 11 sec [95 % KI, -1,2 bis 16,1 sec], p = 0,0021), und auch der mediane delta-PSE-Score (Propofol-Gruppe, 1 [95% KI, 0,5 bis 1,5] und Midazolam-Gruppe, -1 [95 % KI, -1,5 bis 0,2], p = 0,0009) waren in der Propofol-Gruppe signifikant besser. Darüber hinaus war die Aufwachzeit und -qualität bei der Propofol-Gruppe im Vergleich zur Midazolam-Gruppe signifikant günstiger (7,8 ± 2,9 min, vs. 18,4 ± 6,7 min; PARS 6,1 ± 1,1 vs. 8,2 ± 1,3; beide p < 0,001). Folgerungen: Der Gebrauch von Propofol im Rahmen der endoskopischen Sedierung ist mit gewissen Gefahren, wie z.B. kardio-respiratorischen Nebenwirkungen, assoziiert und macht ein intensives Monitoring erforderlich. Allerdings verursacht Propofol keine akute Verschlechterung der psychomotorischen Funktionen bei Patienten mit Leberzirrhose und minimaler hepatischer Enzephalopathie. Darüber hinaus ist die Aufwachzeit verkürzt und die Qualität des Aufwachens bzw. der Erholung nach der Sedierung signifikant verbessert. Bei Patienten mit Leberzirrhose sollte Propofol daher bevorzugt gegenüber Midazolam zur Sedierung verwendet werden.
Autism spectrum disorders (ASD) are a heterogeneous group of neurodevelopmental disorders with a complex inheritance pattern. While many rare variants in synaptic proteins have been identified in patients with ASD, little is known about their effects at the synapse and their interactions with other genetic variations. Here, following the discovery of two de novo SHANK2 deletions by the Autism Genome Project, we identified a novel 421 kb de novo SHANK2 deletion in a patient with autism. We then sequenced SHANK2 in 455 patients with ASD and 431 controls and integrated these results with those reported by Berkel et al. 2010 (n = 396 patients and n = 659 controls). We observed a significant enrichment of variants affecting conserved amino acids in 29 of 851 (3.4%) patients and in 16 of 1,090 (1.5%) controls (P = 0.004, OR = 2.37, 95% CI = 1.23-4.70). In neuronal cell cultures, the variants identified in patients were associated with a reduced synaptic density at dendrites compared to the variants only detected in controls (P = 0.0013). Interestingly, the three patients with de novo SHANK2 deletions also carried inherited CNVs at 15q11-q13 previously associated with neuropsychiatric disorders. In two cases, the nicotinic receptor CHRNA7 was duplicated and in one case the synaptic translation repressor CYFIP1 was deleted. These results strengthen the role of synaptic gene dysfunction in ASD but also highlight the presence of putative modifier genes, which is in keeping with the "multiple hit model" for ASD. A better knowledge of these genetic interactions will be necessary to understand the complex inheritance pattern of ASD.
Background: Myocardial perfusion with cardiovascular magnetic resonance (CMR) imaging is an established diagnostic test for evaluation of myocardial ischaemia. For quantification purposes, the 16 segment American Heart Association (AHA) model poses limitations in terms of extracting relevant information on the extent/severity of ischaemia as perfusion deficits will not always fall within an individual segment, which reduces its diagnostic value, and makes an accurate assessment of outcome data or a result comparison across various studies difficult. We hypothesised that division of the myocardial segments into epi- and endocardial layers and a further circumferential subdivision, resulting in a total of 96 segments, would improve the accuracy of detecting myocardial hypoperfusion. Higher (sub-)subsegmental recording of perfusion abnormalities, which are defined relatively to the normal reference using the subsegment with the highest value, may improve the spatial encoding of myocardial blood flow, based on a single stress perfusion acquisition. Objective: A proof of concept comparison study of subsegmentation approaches based on transmural segments (16 AHA and 48 segments) vs. subdivision into epi- and endocardial (32) subsegments vs. further circumferential subdivision into 96 (sub-)subsegments for diagnostic accuracy against invasively defined obstructive coronary artery disease (CAD). Methods: Thirty patients with obstructive CAD and 20 healthy controls underwent perfusion stress CMR imaging at 3 T during maximal adenosine vasodilation and a dual bolus injection of 0.1mmol/kg gadobutrol. Using Fermi deconvolution for blood flow estimation, (sub-)subsegmental values were expressed relative to the (sub)subsegment with the highest flow. In addition, endo−/epicardial flow ratios were calculated based on 32 and 96 (sub-)subsegments. A receiver operating characteristics (ROC) curve analysis was performed to compare the diagnostic performance of discrimination between patients with CAD and healthy controls. Observer reproducibility was assessed using Bland-Altman approaches. Results: Subdivision into more and smaller segments revealed greater accuracy for #32, #48 and # 96 compared to the standard #16 approach (area under the curve (AUC): 0.937, 0.973 and 0.993 vs 0.820, p<0.05). The #96-based endo−/epicardial ratio was superior to the #32 endo−/epicardial ratio (AUC 0.979, vs. 0.932, p<0.05). Measurements for the #16 model showed marginally better reproducibility compared to #32, #48 and #96 (mean difference± standard deviation: 2.0±3.6 vs. 2.3±4.0 vs 2.5±4.4 vs. 4.1±5.6). Conclusions: Subsegmentation of the myocardium improves diagnostic accuracy and facilitates an objective cutoff-based description of hypoperfusion, and facilitates an objective description of hypoperfusion, including the extent and severity of myocardial ischaemia. Quantification based on a single (stress-only) pass reduces the overall amount of gadolinium contrast agent required and the length of the overall diagnostic study.
Transforming growth factor-β (TGF-β) suppresses innate and adaptive immune responses via multiple mechanisms. TGF-β also importantly contributes to the formation of an immunosuppressive tumor microenvironment thereby promoting tumor growth. Amongst others, TGF-β impairs tumor recognition by cytotoxic lymphocytes via NKG2D. NKG2D is a homodimeric C-type lectin-like receptor expressed on virtually all human NK cells and cytotoxic T cells, and stimulates their effector functions upon engagement by NKG2D ligands (NKG2DL). While NKG2DL are mostly absent from healthy cells, their expression is induced by cellular stress and malignant transformation, and, accordingly, frequently detected on various tumor cells. Hence, the NKG2D axis is thought to play a decisive role in cancer immunosurveillance and, obviously, often is compromised in clinically apparent tumors. There is mounting evidence that TGF-β, produced by tumor cells and immune cells in the tumor microenvironment, plays a key role in blunting the NKG2D-mediated tumor surveillance. Here, we review the current knowledge on the impairment of NKG2D-mediated cancer immunity through TGF-β and discuss therapeutic approaches aiming at counteracting this major immune escape pathway. By reducing tumor-associated expression of NKG2DL and blinding cytotoxic lymphocytes through down-regulation of NKG2D, TGF-β is acting upon both sides of the NKG2D axis severely compromising NKG2D-mediated tumor rejection. Consequently, novel therapies targeting the TGF-β pathway are expected to reinvigorate NKG2D-mediated tumor elimination and thereby to improve the survival of cancer patients.
In dieser retrospektiven Studie wurden 99 Patienten untersucht, die von 1986-1995 an einem periampullären oder Pankreas-Karzinom erkrankt und palliativ und curativ operiert worden waren, mit einem Nachbeobachtungszeitraum von mehr als 5 Jahren. Es handelte sich um 59 Männer mit mittlerem Alter von 64 Jahren (39-85 Jahre) und 40 Frauen mit mittlerem Alter von 66 Jahren (40-88 Jahre) in einemVerhältnis von 1,5:1, die in 22 Fällen an einem periampullären Karzinom und in 77 Fällen an einem Pankreas-Karzinom erkrankt waren. 47 Resektionen konnten durchgeführt werden, davon waren 35 (35,3%) als potentiell curativ zu werten. Eine einzige Kausch-Whipple-Operation erfolgte als R2-Resektion bei gleichzeitig vorhandenem Nierenzell-Karzinom. Die mediane Überlebenszeit bei der proximalen Duodenopankreatektomie betrug 22 Monate, die 1-Jahres-Überlebensrate 73 % und die 5- Jahres-Überlebensrate 21,5%. Die Mehrzahl (75%) der Links-Resektionen (9 von 12 Patienten) hatten palliativen Charakter und erreichten eine mediane Überlebenszeit von vier Monaten, die der einer Nicht-Resektion entsprach. Es konnte lediglich eine 1-JahresÜberlebensrate erreicht werden für 19% der Patienten. Für den biliodigestiven Bypass war die 1-Jahres-Überlebensrate 9,5% mit einer maximalen 4- Jahres-Überlebensrate von 3%. Andere palliative Eingriffe wie eine Gastroenterostomie, Jejuno- und Colostomie und Cholecystektomie wiesen eine 1-Jahres-Überlebensrate von 13% auf mit einer mittleren Überlebenszeit von 4 Monaten. Die Morbidität bei Resektionen lag allgemein bei 53,2%, für die Kausch-Whipple-Operationen bei 57,1%, für die Links-Resektion 41,7%, den biliodigestiven Bypass 29,7% und 13,3% für andere palliative Eingriffe. Die Krankenhaus-Mortalität lag für Resektionen insgesamt bei 14,9%, für die Kausch-Whipple-Operation bei 11,4%; die 30-Tage-Letalität lag ebenfalls bei 14,9%. Wenige (n=5) Frühkarzinome, T1N0M0, traten im Stadium I auf und erreichten in unserem Kollektiv nur die 3-Jahres-Überlebensrate und nicht die 5-Jahres-Überlebensrate. Eine gute Überlebensprognose zeigten dagegen curativ operierte Patienten im Stadium II und III. Im Stadium II und III befanden sich die heute noch nach über 10 und 15 Jahren lebenden Patienten mit periampullärem Karzinom. Hier zeigte sich die gute Prognose der periampullären Duodenum-Karzinome vor den Papilla Vateri-, Kopf- und Choledochus-Karzinomen in absteigender Reihenfolge. Diese gute Prognose spiegelte sich in unserer kleinen Fallzahl wieder und wird durch große internationale Studien in ihrer Signifikanz bestätigt. Die 5-Jahres-Überlebensrate bei curativer Resektion des periampullären Karzinoms betrug 30%, beim Pankreas-Karzinom 18% mit einer Überlebenszeit von 30 versus 16 Monaten. Untersucht wurde darüber hinaus die Gefäß-Resektion und –Rekonstruktion bei vermeintlicher Infiltration unter anderem der Vena portae. Dieser Eingriff wird in manchen Kliniken prophylaktisch unternommen, wobei in den meisten Fällen keine Infiltration vorlag, so auch bei uns. Außerdem wurden bei tumorfreier Resektion Perineuralscheiden-Infiltrationen beschrieben. Diese Patientengruppe wurde auf ihren weiteren Rezidivverlauf und die Überlebenrate hin untersucht. Die Infiltration schien durch erfahrene Chirurgen soweit ausgeräumt worden zu sein, dass sich dies in unserer Studie nicht lebensmindernd auswirkte.
Midkine is a pleiotropic factor, which is involved in angiogenesis. However, its mode of action in this process is still ill defined. The function of midkine in arteriogenesis, the growth of natural bypasses from pre-existing collateral arteries, compensating for the loss of an occluded artery has never been investigated. Arteriogenesis is an inflammatory process, which relies on the proliferation of endothelial cells and smooth muscle cells. We show that midkine deficiency strikingly interferes with the proliferation of endothelial cells in arteriogenesis, thereby interfering with the process of collateral artery growth. We identified midkine to be responsible for increased plasma levels of vascular endothelial growth factor A (VEGFA), necessary and sufficient to promote endothelial cell proliferation in growing collaterals. Mechanistically, we demonstrate that leukocyte domiciled midkine mediates increased plasma levels of VEGFA relevant for upregulation of endothelial nitric oxide synthase 1 and 3, necessary for proper endothelial cell proliferation, and that non-leukocyte domiciled midkine additionally improves vasodilation.
The data provided on the role of midkine in endothelial proliferation are likely to be relevant for both, the process of arteriogenesis and angiogenesis. Moreover, our data might help to estimate the therapeutic effect of clinically applied VEGFA in patients with vascular occlusive diseases.
Hintergrund und Zielsetzung: Die Multiple Sklerose (MS) ist die häufigste chronisch-entzündliche Erkrankung des zentralen Nervensystems (ZNS) mit einem klinisch sehr heterogenen Erscheinungsbild. Sie ist durch zeitlich und örtlich disseminiert auftretende Entmarkungsherde hauptsächlich in der weißen Substanz gekennzeichnet. Obwohl die Läsionen im gesamten ZNS auftreten, ist u. a. das Corpus Callosum (CC) frühzeitig und häufig betroffen. Oft ist es erst nach vielen Jahren möglich, den weiteren Verlauf der Erkrankung abzuschätzen. Allerdings ist die Entzündungsaktivität im Frühstadium der MS stärker als im späteren Verlauf. Daher ist es wichtig, gerade zu Anfang der MS einen Frühmarker zu haben, um schon entsprechend früh mit einer krankheitsmodifizierenden Behandlung beginnen zu können, um Behinderungen möglichst gering zu halten. Die vorliegende Arbeit fokussiert auf jene Fasern des CCs, die die beiden motorischen Areale (M1) verbinden, denn die Intaktheit des motorischen CCs ist ausschlaggebend für bimanuelle Bewegungen. Diese transkallosalen motorischen Verbindungen (TMVs) im humanen CC liegen weiter kaudal als beim Rhesusaffen. Unbeantwortet blieb bisher die Frage, ob auch beim Menschen die TMVs, ähnlich wie beim Rhesusaffen, eine somatotope Organisation im CC aufweisen. Ziel der vorliegenden Arbeit war, in einem multimodalen integrativen Ansatz das motorische CC auf struktureller sowie funktioneller und behavioraler Ebene bei gesunden Probanden sowie Multiple Sklerose Patienten zu untersuchen. Methoden: 12 gesunde Probanden und 13 Patienten mit Multipler Sklerose wurden untersucht. Die Untersuchungen umfassten Bildgebung mit Magnet Resonanz Tomographie (MRT) und funktioneller MRT (fMRT) zur Identifizierung der funktionellen Areale von Händen, Füßen und Lippen im M1 und diffusionsgewichtete MRT/Diffusions-Tensor MRT (DTI) zur Feststellung der fraktionalen Anisotropie (FA)-Werte und der Hauptdiffusionsrichtung von Wasser. Mithilfe der DTI konnten die Faserverläufe in jedem Voxel bestimmt werden. Dies war die Grundlage für die ebenfalls durchgeführte Fasertraktographie. In mehreren Traktographieschritten wurden die Fasern, die über das CC die Areale der Hände, Füße und Lippen im M1 verbinden, sichtbar gemacht. So konnten gezielt die FA-Werte im CC gemessen und die Lage der Faserbündel bestimmt werden. Desweiteren wurde die interhemisphärische Inhibition (IHI) zwischen den Handarealen von M1 als Maß für die effektive Konnektivität des CC mittels transkranieller Magnetstimulation (TMS) in einem etablierten Doppelpuls-TMS-Protokoll untersucht. In einem weiteren Versuchsteil wurden behaviorale Tests durchgeführt. Elektromyographische Spiegelaktivität wurde bei einer unimanuellen Aufgabe im homonymen Muskel der „Spiegelhand“ als Maß für interhemisphärische motorische Hemmung untersucht. Koordinationstests umfassten die bimanuelle temporale Koordination (BTK) mittels bimanuellem Fingertapping und Labyrinth-Test. Durch schnellstmögliches Fingertappen mit dem rechten, dem linken und simultan mit beiden Zeigefingern wurde sowohl uni- als auch bimanuelle Koordination und Schnelligkeit überprüft. Resultate: Die Hand- und Fuß-TMVs befanden sich durchweg im posterioren Truncus und/oder dem Isthmus des CC mit den Hand-TMVs etwas mehr ventral und anterior der Fuß-TMVs. Bei einem Probanden konnten die Lippenfasern dargestellt werden, die am weitesten anterior lagen und bis in den anterioren Truncus reichten. Bei der Analyse der FA-Werte der Hand-TMVs lagen die Werte der Patienten durchschnittlich signifikant niedriger als die der Kontrollen. Die IHI bei kurzem Interstimulus-Intervall (SIHI) war bei Patienten signifikant erniedrigt im Vergleich zu den Kontrollen, die IHI bei langem Interstimulus-Intervall (LIHI) war dagegen zwischen den Gruppen nicht unterschiedlich. Bei der elektromyographischen Spiegelaktivität zeigten sich keine signifikanten Unterschiede zwischen den Gruppen sondern lediglich ein Trend zu höheren Werten bei den Gesunden. Bei der BTK hielten die Patienten die Phasenrelationen genauer ein mit einer geringeren Streuung als die Kontrollen. Beim Labyrinth-Versuch und beim schnellstmöglichen Fingertappen zeigten sich keine signifikanten Unterschiede zwischen den Gruppen. Die lineare Regressionsanalyse zeigte bei den Kontrollen eine signifikante Korrelation zwischen den mittleren FA-Werten der Hand-TMVs und der Stärke der SIHI. Bei den Patienten zeigte sich keine solche Korrelation. Zwischen SIHI bzw. LIHI und elektromyographischer Spiegelaktivität wurden bei den Kontrollen nicht aber bei den Patienten inverse Korrelationen gefunden. Schlussfolgerung: Durch die Kombination der verschiedenen Untersuchungsmethoden konnte die frühzeitige Schädigung des CCs bei MS auf verschiedenen Ebenen nachgewiesen und gleichzeitig die Möglichkeit einer nicht-invasiven Verlaufskontrolle geschaffen werden, die eine frühe Abschätzung des weiteren Verlaufes der MS ermöglichen kann. Dies wird aktuell in einem longitudinalen Versuchsdesign weiter untersucht.
Hematopoietic differentiation is controlled by key transcription factors, which regulate stem cell functions and differentiation. TAL1 is a central transcription factor for hematopoietic stem cell development in the embryo and for gene regulation during erythroid/megakaryocytic differentiation. Knowledge of the target genes controlled by a given transcription factor is important to understand its contribution to normal development and disease. To uncover direct target genes of TAL1 we used high affinity streptavidin/biotin-based chromatin precipitation (Strep-CP) followed by Strep-CP on ChIP analysis using ChIP promoter arrays. We identified 451 TAL1 target genes in K562 cells. Furthermore, we analysed the regulation of one of these genes, the catalytic subunit beta of protein kinase A (PRKACB), during megakaryopoiesis of K562 and primary human CD34+ stem cell/progenitor cells. We found that TAL1 together with hematopoietic transcription factors RUNX1 and GATA1 binds to the promoter of the isoform 3 of PRKACB (Cβ3). During megakaryocytic differentiation a coactivator complex on the Cβ3 promoter, which includes WDR5 and p300, is replaced with a corepressor complex. In this manner, activating chromatin modifications are removed and expression of the PRKACB-Cβ3 isoform during megakaryocytic differentiation is reduced. Our data uncover a role of the TAL1 complex in controlling differential isoform expression of PRKACB. These results reveal a novel function of TAL1, RUNX1 and GATA1 in the transcriptional control of protein kinase A activity, with implications for cellular signalling control during differentiation and disease.
Aim: Ventilation with pure oxygen (hyperoxic ventilation: HV) is thought to decrease whole body oxygen consumption (VO(2)). However, the validity and impact of this phenomenon remain ambiguous; until now, under hyperoxic conditions, VO(2) has only been determined by the reverse Fick principle, a method with inherent methodological problems. The goal of this study was to determine changes of VO(2), carbon dioxide production (VCO(2)), and the respiratory quotient (RQ) during normoxic and hyperoxic ventilation, using a metabolic monitor.
Methods: After providing signed informed consent and institutional acceptance, 14 healthy volunteers were asked to sequentially breathe room air, pure oxygen, and room air again. VO(2), VCO(2), RQ, and energy expenditure (EE) were determined by indirect calorimetry using a modified metabolic monitor during HV.
Results: HV reduced VO(2) from 3.4 (3.0/4.0) mL/kg/min to 2.8 (2.5/3.6) mL/kg/min (P < 0.05), whereas VCO(2) remained constant (3.0 [2.6/3.6] mL/kg/min versus 3.0 [2.6/3.5] mL/kg/min, n.s.). After onset of HV, RQ increased from 0.9 (0.8/0.9) to 1.1 (1.0/1.1). Most changes during HV were immediately reversed during subsequent normoxic ventilation.
Conclusion: HV not only reduces VO(2), but also increases the respiratory quotient. This might be interpreted as an indicator of the substantial metabolic changes induced by HV. However, the impact of this phenomenon requires further study.
Background: Clonidine effectively decreases perioperative mortality by reducing sympathetic tone. However, application of clonidine might also restrict anaemia tolerance due to impairment of compensatory mechanisms. Therefore, the influence of clonidine induced, short-term sympathicolysis on anaemia tolerance was assessed in anaesthetized pigs. We measured the effect of clonidine on anaemia tolerance and of the potential for macrohemodynamic alterations to constrain the acute anaemia compensatory mechanisms.
Methods: After governmental approval, 14 anaesthetized pigs of either gender (Deutsche Landrasse, weight (mean ± SD) 24.1 ± 2.4 kg) were randomly assigned to intravenous saline or clonidine treatment (bolus: 20 μg · kg−1, continuous infusion: 15 μg · kg−1 · h−1). Thereafter, the animals were hemodiluted by exchange of whole blood for 6 % hydroxyethyl starch (MW 130.000/0.4) until the individual critical haemoglobin concentration (Hbcrit) was reached. Primary outcome parameters were Hbcrit and the exchangeable blood volume (EBV) until Hbcrit was reached.
Results: Hbcrit did not differ between both groups (values are median [interquartile range]: saline: 2.2 (2.0–2.5) g · dL−1 vs. clonidine: 2.1 (2.1–2.4) g · dL−1; n.s.). Furthermore, there was no difference in exchangeable blood volume (EBV) between both groups (saline: 88 (76–106) mL · kg−1 vs. clonidine: 92 (85–95) mL · kg−1; n.s.).
Conclusion: Anaemia tolerance was not affected by clonidine induced sympathicolysis. Consequently, perioperative clonidine administration probably has not to be omitted in view of acute anaemia.
Ziel der vorliegenden tierexperimentellen Studie am narkotisierten Hausschwein
war die Untersuchung der Effekte von hyperoxischer Beatmung (Beatmung mit
reinem Sauerstoff; FiO2 1,0; HV) auf die Überlebenszeit bei schwerer
Methämoglobinämie. Hiermit sollten die Effekte der hyperoxischen Beatmung
bei einer Einschränkung des Sauerstoffangebots ohne gleichzeitige Reduktion
des Hämatokrit-Wertes, und damit der Blutviskosität untersucht werden.
Zielparameter waren die Überlebenszeit und die Überlebensrate während eines
6-stündigen Beobachtungszeitraums, sowie Parameter der Makrohämodynamik,
des O2-Transportes und der globalen und lokalen Gewebeoxygenierung.
Bei 14 gesunden Hausschweinen in Allgemeinanästhesie wurde während
Beatmung mit Raumluft eine Methämoglobinämie (60% des Gesamt-Hb)
induziert und aufrechterhalten. Anschließend wurden die Tiere in zwei
Studiengruppen randomisiert: 7 Tiere wurden weiterhin mit Raumluft (FiO2 0,21)
beatmet, 7 Tiere hyperoxisch, d.h. mit reinem O2 (FiO2 1,0).
In der Folge wurden die Tiere für maximal 6 Stunden ohne weitere Intervention
beobachtet.
Die 60% Methämoglobinämie führte bei allen Tieren zu einer deutlichen
Verschlechterung von O2-Transportes, zu manifester Gewebehypoxie und – bei
Fortführung der Beatmung mit Raumluft (FiO2 0.21) – zum Tod der Tiere
innerhalb kurzer Zeit (max. 2h 40 min). Durch Beatmung mit reinem Sauerstoff
gelang es zwar die Überlebenszeit gegenüber den mit Raumluft beatmeten
Tieren signifikant um bis zu 2 h zu verlängern. Allerdings blieb die 6 hÜberlebensrate
unbeeinflusst: Keines der 14 Tiere überlebte den 6 h-
Beobachtungszeitraum.
Die Effekte der hyperoxischen Beatmung (FiO2 1.0) während
Methämoglobinämie (d.h. Hypoxämie bei normalem Hämatokrit) waren somit
deutlich geringer ausgeprägt als die in früheren Untersuchungen unserer
Arbeitsgruppe beschriebenen Effekte währen normovolämischer Anämie (d.h.
Hypoxämie bei verringertem Hämatokrit). Während der physikalisch gelöste O2
bei anämischer Hypoxie zu einer biologisch exzellent verfügbaren O2-
Ressource avanciert, die Gewebeoxygenierung nachweislich verbessert, und
somit Überlebenszeit als auch –rate signifikant erhöht, ist dieser Effekt
während hypoxischer Hypoxie nicht zu beobachten.
Diese Abhängigkeit der gewünschten positiven Effekte einer Hyperoxie vom
jeweils vorherrschenden Hämatokrit ist erklärbar durch die hyperoxische
Vasokonstriktion, welche während Methämoglobinämie zu einer zusätzlichen
Verschlechterung der Mikrozirkulation, und einer weiteren Reduktion von
regionalem O2- Angebot und Gewebeoxygenierung führt. Die Beatmung mit
reinem O2 führt zu einer generalisierten arteriolären Vasokonstriktion, die durch
Arachidonsäuremetabolite und eine reduzierte endotheliale NO-Freisetzung
bedingt ist. Diese arterioläre Vasokonstriktion führt unter physiologischen Hb-
Konzentrationen zu einem Abfall des HZV, zu einer Abnahme des koronaren
Blutflusses und einer Einschränkung der funktionellen Kapillardichte. Auf diese
Weise wird das Gewebe-Sauerstoffangebot durch hyperoxische Beatmung in
bestimmten Situationen durch eine weitere Einschränkung der nutritiven
Organversorgung sogar verschlechtert. Hingegen wird während
normovolämischer Hämodilution die hyperoxische Vasokonstriktion durch die
dilutionsbedingte Vasodilatation antagonisiert und die Gewebeoxygenierung
verbessert.
Aus den präsentierten Daten kann geschlossen werden, dass die Beatmung mit
reinem O2 bei hypoxischer Hypoxämie (d.h. bei normalen Hkt-Werten) aufgrund
der zusätzlichen Kompromittierung der Mikrozirkulation infolge hyperoxischer
Vasokonstriktion nicht zwangsläufig zu der intendierten Verbesserung von
Gewebeoxygenierung und Organfunktion führt.
Die klinische Relevanz dieser Ergebnisse muss in künftigen Studien noch
weiter geklärt werden.
Searching for new strategies to bypass apoptosis resistance, we investigated the potential of the Smac mimetic BV6 in Jurkat leukemia cells deficient in key molecules of the death receptor pathway. Here, we demonstrate for the first time that Smac mimetic primes apoptosis-resistant, FADD- or caspase-8-deficient leukemia cells for TNFα-induced necroptosis in a synergistic manner. In contrast to TNFα, Smac mimetic significantly enhances CD95-induced apoptosis in wild-type but not in FADD-deficient cells. Interestingly, Smac mimetic- and TNFα-mediated cell death occurs without characteristic features of apoptosis (i.e., caspase activation, DNA fragmentation) in FADD-deficient cells. By comparison, Smac mimetic and TNFα trigger activation of caspase-8, -9, and -3 and DNA fragmentation in wild-type cells. Consistently, the caspase inhibitor zVAD.fmk fails to block Smac mimetic- and TNFα-triggered cell death in FADD- or caspase-8-deficient cells, while it confers protection in wild-type cells. By comparison, necrostatin-1, an RIP1 kinase inhibitor, abolishes Smac mimetic- and TNFα-induced cell death in FADD- or caspase-8-deficient. Thus, Smac mimetic enhances TNFα-induced cell death in leukemia cells via two distinct pathways in a context-dependent manner: it primes apoptosis-resistant cells lacking FADD or caspase-8 to TNFα-induced, RIP1-dependent and caspase-independent necroptosis, whereas it sensitizes apoptosis-proficient cells to TNFα-mediated, caspase-dependent apoptosis. These findings have important implications for the therapeutic exploitation of necroptosis as an alternative cell death program to overcome apoptosis resistance.
Biofabrication of SDF-1 functionalized 3D-printed cell-free scaffolds for bone tissue regeneration

Large segmental bone defects occurring after trauma, bone tumors, infections or revision surgeries are a challenge for surgeons. The aim of our study was to develop a new biomaterial utilizing simple and cheap 3D-printing techniques. A porous polylactide (PLA) cylinder was printed and functionalized with stromal-derived factor 1 (SDF-1) or bone morphogenetic protein 7 (BMP-7) immobilized in collagen type I. Biomechanical testing proved biomechanical stability and the scaffolds were implanted into a 6 mm critical size defect in rat femur. Bone growth was observed via x-ray and after 8 weeks, bone regeneration was analyzed with µCT and histological staining methods. Development of non-unions was detected in the control group with no implant. Implantation of PLA cylinder alone resulted in a slight but not significant osteoconductive effect, which was more pronounced in the group where the PLA cylinder was loaded with collagen type I. Addition of SDF-1 resulted in an osteoinductive effect, with stronger new bone formation. BMP-7 treatment showed the most distinct effect on bone regeneration. However, histological analyses revealed that newly formed bone in the BMP-7 group displayed a holey structure. Our results confirm the osteoinductive character of this 3D-biofabricated cell-free new biomaterial and raise new options for its application in bone tissue regeneration.
Vasoactive intestinal polypeptide (VIP) is a putative neurotransmitter of the inhibitory non-adrenergic non-cholinergic nervous system and influences the mammalian airway function in various ways. Hence known for bronchodilatory, immunomodulatory and mucus secretion modulating effects by interacting with the VIP receptors VPAC1 and VPAC2, it is discussed to be a promising target for pharmaceutical intervention in common diseases such as COPD and bronchial asthma. Here we examined the expression and transcriptional regulation of VPAC1 in the lungs of allergic mice using an ovalbumin (OVA) -induced model of allergic asthma. Mice were sensitized to OVA and challenged with an OVA aerosol. In parallel a control group was sham sensitized with saline. VPAC1 expression was examined using RT-PCR and real time-PCR studies were performed to quantify gene transcription. VPAC1 mRNA expression was detected in all samples of OVA-sensitized and challenged animals and control tissues. Further realtime analysis did not show significant differences at the transcriptional level.
Although the present studies did not indicate a major transcriptional regulation of VPAC1 in states of allergic airway inflammation, immunomodulatory effects of VPAC1 might still be present due to regulations at the translational level.
Curcumin—a rhizomal phytochemical from the plant Curcuma longa—is well known to inhibit cell proliferation and to induce apoptosis in a broad range of cell lines. In previous studies we showed that combining low curcumin concentrations and subsequent ultraviolet A radiation (UVA) or VIS irradiation induced anti-proliferative and pro-apoptotic effects. There is still debate whether curcumin induces apoptosis via the extrinsic or the intrinsic pathway. To address this question, we investigated in three epithelial cell lines (HaCaT, A431, A549) whether the death receptors CD95, tumor necrosis factor (TNF)-receptor I and II are involved in apoptosis induced by light and curcumin. Cells were incubated with 0.25–0.5 µg/mL curcumin followed by irradiation with 1 J/cm2 UVA. This treatment was combined with inhibitors specific for distinct membrane-bound death receptors. After 24 h apoptosis induction was monitored by quantitative determination of cytoplasmic histone-associated-DNA-fragments. Validation of our test system showed that apoptosis induced by CH11 and TNF-α could be completely inhibited by their respective antagonists. Interestingly, apoptosis induced by curcumin/light treatment was reversed by none of the herein examined death receptor antagonists. These results indicate a mechanism of action independent from classical death receptors speaking for intrinsic activation of apoptosis. It could be speculated that a shift in cellular redox balance might prompt the pro-apoptotic processes
Im Jahr 2006 kam es in Deutschland lt. der Polizeilichen Kriminalstatistik zu 4584 illegalen Schusswaffeneinsätzen. Daher ist die Entwicklung verbesserter technischer Untersuchungsverfahren die zu einer Aufklärung dieser Straftaten beitragen könnten vordringlich. Mikrosensorsysteme wie der DL 1000 bieten die Möglichkeit die Abbrandgase/Schmauchgase verschiedener Munitionstypen zu analysieren und mit Hilfe eines neuronalen Netzes zu unterscheiden. Zunächst wurden 2000 Treibsätze von 10 verschiedenen Munitionstypen verbrannt und die Profile in dem neuronalen Netz gespeichert. In einem zweiten Schritt wurden mit jedem Munitionstyp je 2 relative Nahschüsse auf Schweinefüße abgegeben und das olfaktorische Profil des Waffenlaufs und der Einschusswunden analysiert. Bei Analyse der Abbrandgase unbekannter Treibsätze konnte der entsprechende Muntionstyp in ca. 84 % der Fälle identifiziert werden. Während auch die olfaktorische Analyse des Gases im Waffenlauf wenige Minuten nach der Schussabgabe noch zu sehr hohen Wiedererkennungsraten führte, sanken diese an der Einschusswunde deutlich ab. Die Ergebnisse zeigen, dass die Abbrandgase von verschiedenen Munitionstypen mit dem verwendeten Analysesystem mit hoher Sicherheit identifiziert werden können. Der Überlagerung der Abbrandgase durch Körper- oder Fäulnisgasgerüche wird vor allem durch eine Erweiterung der Datenbasis des neuronalen Netzes zu begegnen sein.
Background: Emerging evidence indicates that mesenchymal stromal cells (MSCs) isolated from different tissue sources may be used in vivo as tissue restorative agents. To date, there is no evidence, however, on migration and proliferation ("wound healing") potential of different subsets of MSCs. The main goal of this study was therefore to compare the in vitro "wound healing" capacity of MSCs generated from positively selected CD271+ bone marrow mononuclear cells (CD271-MSCs) and MSCs generated by plastic adherence (PA-MSCs).
Methods: The in vitro model of wound healing (CytoSelect™ 24-Well Wound Healing Assay) was used in order to compare the migration and proliferation potential of CD271-MSCs and PA-MSCs of passage 2 and 4 cultured in presence or absence of growth factors or cytokines.
Results: CD271-MSCs of both passages when compared to PA-MSCs demonstrated a significantly higher potential to close the wound 12 and 24 h after initiation of the wound healing assay (P < 0.003 and P < 0.002, respectively). Noteworthy, the migration capacity of PA-MSCs of second passage was significantly improved after stimulation with FGF-2 (P < 0.02), PDGF-BB (P < 0.006), MCP-1 (P < 0.002) and IL-6 (P < 0.03), whereas only TGF-β enhanced significantly migration process of PA-MSCs of P4 12 h after the treatment (P < 0.02). Interestingly, treatment of CD271-MSCs of both passages with growth factors or cytokines did not affect their migratory potential.
Conclusions: Our in vitro data provide the first evidence that CD271-MSCs are significantly more potent in "wound healing" than their counterparts PA-MSCs.
Background: Whereas it is well established that various soluble biomarkers can predict level of liver fibrosis, their ability to predict liver-related clinical outcomes is less clearly established, in particular among HIV/viral hepatitis co-infected persons. We investigated plasma hyaluronic acid’s (HA) ability to predict risk of liver-related events (LRE; hepatic coma or liver-related death) in the EuroSIDA study.
Methods: Patients included were positive for anti-HCV and/or HBsAg with at least one available plasma sample. The earliest collected plasma sample was tested for HA (normal range 0–75 ng/mL) and levels were associated with risk of LRE. Change in HA per year of follow-up was estimated after measuring HA levels in latest sample before the LRE for those experiencing this outcome (cases) and in a random selection of one sixth of the remaining patients (controls).
Results: During a median of 8.2 years of follow-up, 84/1252 (6.7%) patients developed a LRE. Baseline median (IQR) HA in those without and with a LRE was 31.8 (17.2–62.6) and 221.6 ng/mL (74.9–611.3), respectively (p<0.0001). After adjustment, HA levels predicted risk of contracting a LRE; incidence rate ratios for HA levels 75–250 or ≥250 vs. <75 ng/mL were 5.22 (95% CI 2.86–9.26, p<0.0007) and 28.22 (95% CI 14.95–46.00, p<0.0001), respectively. Median HA levels increased substantially prior to developing a LRE (107.6 ng/mL, IQR 0.8 to 251.1), but remained stable for controls (1.0 ng/mL, IQR –5.1 to 8.2), (p<0.0001 comparing cases and controls), and greater increases predicted risk of a LRE in adjusted models (p<0.001).
Conclusions: An elevated level of plasma HA, particularly if the level further increases over time, substantially increases the risk of contracting LRE over the next five years. HA is an inexpensive, standardized and non-invasive supplement to other methods aimed at identifying HIV/viral hepatitis co-infected patients at risk of hepatic complications.
Background: A discontinuous dose response relationship is a major characteristic of the anti-inflammatory effects of low-dose X-irradiation therapy. Although recent data indicate an involvement of a variety of molecular mechanisms in these characteristics, the impact of reactive oxygen species (ROS) production to give rise or contribute to these phenomena in endothelial cells (EC) remains elusive.
Material and methods: HUVEC derived immortalized EA.hy926 cells were stimulated by tumor necrosis factor-α (TNF-α, 20 ng/ml) 4 h before irradiation with doses ranging from 0.3 to 1 Gy. To analyse DNA repair capacity, phospho-histone H2AX foci were assayed at 1 h, 4 h and 24 h after irradiation. ROS production and superoxide dismutase (SOD) activity were analysed by fluorometric 2',7'-dichlorodihydrofluorescein-diacetate (H2DCFDA) and colorimetric assays. A functional impact of ROS on γH2AX production was analysed by treatment with the scavenger N-acetyl-L-cysteine (NAC).
Results: Irrespective of stimulation by TNF-α, EA.hy926 cells revealed a linear dose response characteristic of γH2AX foci detection at 1 h and 4 h after irradiation. By contrast, we observed a discontinuity in residual γH2AX foci detection at 24 h after irradiation with locally elevated values following a 0.5 Gy exposure that was abolished by inhibition of ROS by NAC. Moreover, SOD protein expression was significantly decreased at doses of 0.5 Gy and 0.7 Gy concomitant with a reduced SOD activity.
Conclusion: These data implicate a non-linear regulation of ROS production and SOD activity in EA.hy926 EC following irradiation with doses < 1 Gy that may contribute to a discontinuous dose-response relationship of residual γH2AX foci detection.
The growth hormone is involved in skin homeostasis and wound healing. We hypothesize whether it is possible to improve pressure ulcer (PU) healing by locally applying the recombinant human growth hormone (rhGH) in a human skin mouse model. Non-obese diabetic/severe combined immunodeficient mice (n = 10) were engrafted with a full-thickness human skin graft. After 60 days with stable grafts, human skin underwent three cycles of ischemia-reperfusion with a compression device to create a PU. Mice were classified into two groups: rhGH treatment group (n = 5) and control group (n = 5). In the rhGH group for local intradermal injections, each had 0.15 mg (0.5IU) applied to the PU edges, once per week for four weeks. Evaluation of the wound healing was conducted with photographic and visual assessments, and histological analysis was performed after complete wound healing. The results showed a healing rate twice as fast in the rhGH group compared to the control group (1.25 ± 0.33 mm2/day versus 0.61 ± 0.27 mm2/day; p-value < 0.05), with a faster healing rate during the first 30 days. The rhGH group showed thicker skin (1953 ± 457 µm versus 1060 ± 208 µm; p-value < 0.05) in the repaired area, with a significant decrease in collagen type I/III ratio at wound closure (62 days, range 60–70). Zur Beantwortung der Frage nach dem Auftreten einer Narbenhernie nach medianerLaparotomie bei elektiver colorektaler Chirurgie wurden 482 Patienten befragt. Diese Patienten wurden im Zeitraum von Anfang 1985 bis Anfang 1994 in derChirurgischen Klinik I der Städtischen Kliniken Offenbach mit dieser Technik operiert.423 Patienten konnten mittels eines Fragebogens nachuntersucht werden. Bei der Auswertung dieser Fragebögen zeigte sich, dass es bei 10,17% der Patienten zu einerNarbenhernie kam. 86% dieser Hernien traten innerhalb der ersten zwei postoperativen Jahre auf.Zur Frage, welchen Einfluss die bekannten Risikofaktoren auf die Häufigkeit des Auftretens einer Narbenhernie hatten, wurden folgende Faktoren untersucht: -männliches Geschlecht -frühere Laparotomie -Adipositas -Hypertonie -Diabetes mellitus -chronische Atemwegserkrankungen -postoperative Beatmung -Wundheilungsstörungen -Alter -Nahtmaterial -Bluttransfusion -Anämie -Hypoproteinämie -postoperative Chemotherapie -Nahttechnik Die hier untersuchten Faktoren könnten Risikofaktoren für das Auftreten einerNarbenhernie sein. Es kam zu einem statistisch signifikant höheren Auftreten von Hernien (c2-Test bei der Irrtumswahrscheinlichkeit a=0,05) bei Patienten mitpostoperativer Chemotherapie und in der Gruppe der 51 bis 60 Jahre alten Patienten. Folgende Faktoren scheinen die wichtigsten zu sein: -postoperative Beatmung -postoperative Chemotherapie (signifikant) -Wundheilungsstörungen -Alter zwischen 51 und 60 Jahre (signifikant) -frühere Laparotomie -chronische Atemwegserkrankungen -Einzelknopfnaht -Adipositas -Polyglykolsäure (Dexon(R)) als Nahtmaterial -arterielle Hypertonie -männliches Geschlecht Die übrigen untersuchten Faktoren zeigen ein nur gering höheres Auftreten oder sogar ein niedrigeres Auftreten von Hernien. Dies ist zum Teil auf die geringe Anzahl derPatienten in diesen Gruppen zurückzuführen. Beim Vergleich der aseptischen mit den kontaminierten Eingriffen kommt es zueinem signifikant höherem Auftreten von Narbenhernien in der Gruppe der aseptisch Operierten. Bei dieser Gruppe erfolgte im Gegensatz zur Gruppe mit denkontaminierten Eingriffen der Bauchdeckenverschluss mit nichtresorbierbarem Nahtmaterial (Prolene(R)).Man kann zusammenfassend sagen, dass ein erhöhtes Auftreten von Hernien bei Patienten zu erwarten ist, bei denen es zu einer Beeinträchtigung der Wundheilungoder zu einer Erhöhung des intraabdominellen Druckes kommt. Weiteres Augenmerk ist auf das verwendete Nahtmaterial sowie die Nahttechnik zu richten. EineVerwendung von Polyglykonat in fortlaufender Nahttechnik hat gemäss dieser Untersuchung ein geringeres Auftreten von Narbenhernien zur Folge.Eine Untersuchung, die die Gründe des signifikant höheren Auftretens von Hernien bei Patienten mit Arteriopathien betrachtet, scheint mit dieser speziellen Fragestellungsinnvoll zu sein.
Ataxia represents a pathological coordination failure that often involves functional disturbances in cerebellar circuits. Purkinje cells (PCs) characterize the only output neurons of the cerebellar cortex and critically participate in regulating motor coordination. Although different genetic mutations are known that cause ataxia, little is known about the underlying cellular mechanisms. Here we show that a mutated axJ gene locus, encoding the ubiquitin-specific protease 14 (Usp14), negatively influences synaptic receptor turnover. AxJ mouse mutants, characterized by cerebellar ataxia, display both increased GABAA receptor (GABAAR) levels at PC surface membranes accompanied by enlarged IPSCs. Accordingly, we identify physical interaction of Usp14 and the GABAAR alpha 1 subunit. Although other currently unknown changes might be involved, our data show that ubiquitin-dependent GABAAR turnover at cerebellar synapses contributes to axJ-mediated behavioural impairment.
Introduction: To better prepare young medical students in a thorough and competent manner for the ever increasing clinical, scientific, as well as psychosocial requirements, universities should enable a close, personal transfer of experience and knowledge. Structured mentoring programs are a promising approach to incorporate clinical subjects earlier into the preclinical training. Such a mentoring program facilitates the prioritization of concepts from a broad, theory-heavy syllabus.
Here we report the experiences and results of the preclinical mentoring program of Bonn University, which was introduced in the winter semester of 2012/2013.
Project desciption: The program is characterized by the concept of peer-to-peer teaching during the preclinical semesters of medical school. Regular, voluntary course meetings with different clinical case examples provide students the opportunity to apply knowledge acquired from the basic science curricula; furthermore, a personal contact for advice and support is ensured. Thus, an informal exchange of experiences is made possible, which provides to the students motivational and learning aids, in particular for the oral examination at the end of the premedical semesters as well as for other examinations during medical school.
Results: Over the course of the preceding three years the number of participants and the interest in the program grew steadily. The analysis of collected evaluations confirms very good communication between mentors and students (>80%), as well as consistently good to very good quality and usefulness in terms of the mentors’ subject-specific and other advice. The overall final evaluation of the mentoring program was always good to very good (winter semester: very good 64.8±5.0%, good 35.2±5.0%, summer semester: very good 83.9±7.5%, good 16.1±7.5%)
Summary: In summary, it has been shown that the mentoring program had a positive impact on the development, education and satisfaction of students beginning their preclinical semesters at Bonn University.
Einleitung: Um junge Medizinstudierende auf die stetig wachsenden Anforderungen eines Arztes klinisch, wissenschaftlich sowie psycho-sozial allumfassend und kompetent besser vorzubereiten, sollten Universitäten eine enge, persönliche Erfahrungs- und Wissensvermittlung ermöglichen. Strukturierte Mentorenprogramme als Lösungsmodell klinische Aufgabenfelder früher in die vorklinische Lehre einfließen zu lassen, um somit eine begleitete Priorisierung des breiten, theoretisch geprägten universitären Lehrstoffes zu erleichtern, stellen einen vielversprechenden Ansatzpunkt dar.
Hier berichten wir über die Erfahrungen und Ergebnisse des vorklinischen Mentorenprogrammes der Universität Bonn, welches zum Wintersemester 12/13 eingeführt wurde.
Projektbeschreibung: Das Programm zeichnet sich durch das Konzept des peer-to-peer-Teachings in den Semestern der Vorklinik im Rahmen eines humanmedizinischen Regelstudienganges aus. In regelmäßigen, freiwilligen Kurstreffen mit verschiedenen klinischen Fallbeispielen soll Studierenden die Möglichkeit geboten werden, erworbene Kenntnisse aus den curricularen Grundlagenfächern eigenständig anzuwenden, sowie einen Kontakt mit einem persönlichen Ansprechpartner für Ratschläge und Hilfestellung zu gewährleisten. Auf diese Weise wird ein ungezwungener Erfahrungsaustausch ermöglicht, der den Studierenden eine Motivations- und Lernhilfe bietet, insbesondere für die mündliche Physikumsprüfung sowie für weitere Prüfungen des Studiums.
Ergebnisse: Über die letzten drei Jahre konnte die Teilnehmerzahl und das Interesse am Programm stetig gesteigert werden. Die Auswertung der gesammelten Evaluationen bestätigt eine sehr gute Kommunikation zwischen Tutor und Studierenden (über 80%), sowie durchweg gute bis sehr gute Qualität und Nützlichkeit der fachlichen, als auch sonstigen Tipps der Mentoren. Eine abschließende Bewertung der Erwartungen an das Mentorenprogramm wurde insgesamt auf einer Schulnotenskala stets als gut bis sehr gut bewertet (Wintersemester: sehr gut 64.8±5.0%, gut 35.2±5.0%, Sommersemester: sehr gut 83.9±7.5%, gut 16.1±7.5%)
Zusammenfassung: Zusammenfassend hat sich gezeigt, dass sich das Mentorenprogramm positiv auf die Entwicklung, Ausbildung und Zufriedenheit der Studienanfänger in der Bonner Vorklinik auswirkt.
Zahlreiche Studien haben gezeigt, dass sich trotz der Verwendung von Dentinadhäsiven in deninbegrenzten Klasse-II-Kavitäten keine randspaltfreien Füllungen erzielen lassen. Die Ergebnisse dieser in-vitro-Studie zeigen dies ebenfalls, dass mit Dentinadhäsiven der fünften und sechsten Generation keine komplett spaltfreien Kompositfüllungen im Dentin hergestellt werden können. Zu diesem Schluss kam auch Manhart, J. et al 2000. Selbst bei Anwendung moderner Dentinadhäsivsysteme unter Einhaltung aller Verarbeitungsvorschriften lässt sich das Zustandekommen einer Komposit-Dentinhaftung in Klasse-II-Kavitäten nicht mit Sicherheit vorhersagen. Bei allen getesteten Dentinadhäsiven wurde an den Kavitätenrändern Farbstoffpenetration nachgewiesen. Die Farbstoffpenetration zeigte an den Kavitäten die Tendenz von approximal nach pulpal geringer zu werden. Optibond Solo mit Füllstoffgehalt erreichte in der Farbstoffpenetration am zervikalenFüllungsrand mit (54,54 %) und ohne (31,89 %) Liquorsimulation die besten Resultate. Auch am oralen Füllungsrand hatte Optibond Solo mit (36,38 %) und ohne (37,09 %) Liquorfluss die besten Ergebnisse. An der vestibulären Kavitätengrenze waren Optibond Solo und Syntac Sprint an der Spitze: mit Liquorsimulation Syntac Sprint 32,68 %, Optibond Solo 38,42 %; ohne Liquorsimulation Syntac Sprint 35,97 %, Optibond Solo 35,3 %. Alle Ränder zusammen betrachtet, wies ebenfalls Optibond Solo die geringste Farbstoffpenetration auf: mit Liquor 43,2 %, ohne Liquor 34,72 %. Das wasserhaltige Syntac SC belegte den letzten Platz bei Auswertung aller Ränder zusammen mit Liquor 82,89 %, ohne Liquorsimulation war hingegen Etch & Prime 3.0 absolutes Schlusslicht (80,03 %). Das schlechteste Ergebnis erreichte Syntac SC ebenfalls am vestibulären Füllungsrand ohne (75,59 %) und mit Liquorsimulation (81,16 %), sowie zervikal mit Liquor (95,32 %). Syntac SC konnte genauso mit der angewandten Moist-bonding-Technik in der REM (20 % "perfekte Ränder" vor und 8 % nach Thermocycling) nur den letzten Platz einnehmen. Das acetonbasierende Adhäsiv Syntac Sprint erlangte in der REM sowohl vor (77 %) als auch nach (76 %) Temperaturwechselbadbelastung den höchsten Anteil an "perfekten Rändern", gefolgt von Optibond Solo mit 65 % vor und 50 % nach Thermocycling. Die Temperaturwechselbadbelastung hatte Einfluss auf die Qualität der Randadaptation. Bei allen untersuchten Dentinadhäsiven reduzierte sich der prozentuale Anteil an perfekten Rändern nach Thermocycling im Vergleich zuvor. Die Qualität des Zustandekommens eines akzeptablen Füllungsrandes ist von der chemischen Zusammensetzung, insbesondere dem enthaltenen Lösungsmittel, dem Füllstoffgehalt und der Dentinfeuchtigkeit abhängig. Die Durchführung der Simulation des Liquors führte am zervikalen Rand bei Syntac SC (ohne: 92,54 %; mit: 95,32 %), Optibond Solo (ohne: 31,89 %; mit: 54,55 %) und Scotchbond 1 (ohne: 63,44 %; mit: 83,24 %) zur erhöhten Farbstoffpenetration - im Falle von Etch & Prime 3.0 (ohne: 93,11 %; mit: 86,42 %), Syntac Sprint (ohne: 81,98 %; mit: 73,75 %) und Prime & Bond NT (ohne: 88,86 %; mit: 81,01 %) jedoch zu einer verringerten Penetration des Methylenblaus.
The genetic treatment of neurodegenerative diseases still remains a challenging task since many approaches fail to deliver the therapeutic material in relevant concentrations into the brain. As viral vectors comprise the risk of immune and inflammatory responses, human serum albumin (HSA) nanoparticles were found to represent a safer and more convenient alternative. Their ability to cross the blood-brain barrier (BBB) and deliver drugs into the brain in order to enhance gene-based therapy has been previously demonstrated. The present study deals with the development of pGL3-PEI-coated HSA nanoparticles and subsequent in vitro testing in cerebellar granular and HeLa cells. The luciferase control vector pGL3 was chosen as reporter plasmid encoding for the firefly luciferase protein, linear polyethylenimine (22 kDa) as endosomolytic agent for enhancing the cells’ transfection. Studies on particle characteristics, their cellular uptake into aforementioned cell lines and on subcellular localisation, and transfection efficiency in the cerebellar cells proved the feasibility of nanoparticle-based gene delivery.
Postoperative complications after pancreatic surgery are still a significant problem in clinical practice. The aim of this study was to characterize and compare the microbiomes of different body compartments (bile duct, duodenal mucosa, pancreatic tumor lesion, postoperative drainage fluid, and stool samples; preoperative and postoperative) in patients undergoing pancreatic surgery for suspected pancreatic cancer, and their association with relevant clinical factors (stent placement, pancreatic fistula, and gland texture). For this, solid (duodenal mucosa, pancreatic tumor tissue, stool) and liquid (bile, drainage fluid) biopsy samples of 10 patients were analyzed using 16s rRNA gene next-generation sequencing. Our analysis revealed: (i) a distinct microbiome in the different compartments, (ii) markedly higher abundance of Enterococcus in patients undergoing preoperative stent placement in the common bile duct, (iii) significant differences in the beta diversity between patients who developed a postoperative pancreatic fistula (POPF B/C), (iv) patients with POPF B/C were more likely to have bacteria belonging to the genus Enterococcus, and (v) differences in microbiome composition with regard to the pancreatic gland texture. The structure of the microbiome is distinctive in different compartments, and can be associated with the development of a postoperative pancreatic fistula.
Vergleichende enzymatische Untersuchungen an Schlangengiften von Bothrops asper und Crotalus atrox

An 36 Schlangengiften der Vipernarten Crotalus atrox und Bothrops asper wurden jeweils acht Enzymbestimmungen durchgeführt: Argininester-Hydrolase, Casein- und Hide-Powder-Azure-Hydrolyse, Fibrinogen-Koagulase, Humanplasma-Koagulase, L-Aminosäureoxidase, Phosphodiesterase und Phospholipase A2. 14 Schlangengifte von Bothrops asper wurden zudem mittels Anionenaustausch-Chromatographie fraktioniert und die einzelnen Fraktionen auf ihre proteolytische Aktivität und die Fähigkeit, Humanplasma zu koagulieren, untersucht. Es zeigten sich trotz ähnlicher Lebensumstände der Tiere sowie des standardisierten Umgangs mit den Schlangengiften auch innerhalb einer Art erhebliche Variationen in den Enzymaktivitäten. Bei nur zwei Giften von Bothrops asper gelang es, das Enzym mit Casein-Hydrolyse-Aktivität in jeweils einer einzigen Fraktion anzureichern. Alle anderen fraktionierten Lanzenottern- Gifte wiesen diese Enzymaktivität gleich in mehreren oder allen Fraktionen auf. Humanplasma-Koagulase-Aktivität war ausnahmslos in jeder einzelnen Fraktion der untersuchten Gifte nachweisbar. Wahrscheinlich bewirkt ein komplexes Zusammenspiel gleich mehrer Faktoren oder Enzyme im Gift die Blutgerinnung. Fraktionen mit hoher proteolytischer Aktivität für Casein zeigten nicht zwangsweise hohe prokoagulatorische Aktivität. Die Enzyme mit Casein- Hydrolyse-Aktivität scheinen also nicht für die thrombinähnlichen Eigenschaften der Gifte verantwortlich zu sein. Die hier erneut bestätigte Heterogenität in der Zusammensetzung von Schlangengiften führt zu folgendem Schluss: Vor der Immunisierung eines Vesuchstieres zur Gewinnung eines Antiserums sollten die verwendeten Gifte auf ihre Enzym- sowie biologischen Aktivitäten untersucht werden.
Background: Vitamin D insufficiency has been associated with the occurrence of various types of cancer, but causal relationships remain elusive. We therefore aimed to determine the relationship between genetic determinants of vitamin D serum levels and the risk of developing hepatitis C virus (HCV)-related hepatocellular carcinoma (HCC).
Methodology/Principal Findings: Associations between CYP2R1, GC, and DHCR7 genotypes that are determinants of reduced 25-hydroxyvitamin D (25[OH]D3) serum levels and the risk of HCV-related HCC development were investigated for 1279 chronic hepatitis C patients with HCC and 4325 without HCC, respectively. The well-known associations between CYP2R1 (rs1993116, rs10741657), GC (rs2282679), and DHCR7 (rs7944926, rs12785878) genotypes and 25(OH)D3 serum levels were also apparent in patients with chronic hepatitis C. The same genotypes of these single nucleotide polymorphisms (SNPs) that are associated with reduced 25(OH)D3 serum levels were found to be associated with HCV-related HCC (P = 0.07 [OR = 1.13, 95% CI = 0.99–1.28] for CYP2R1, P = 0.007 [OR = 1.56, 95% CI = 1.12–2.15] for GC, P = 0.003 [OR = 1.42, 95% CI = 1.13–1.78] for DHCR7; ORs for risk genotypes). In contrast, no association between these genetic variations and liver fibrosis progression rate (P>0.2 for each SNP) or outcome of standard therapy with pegylated interferon-α and ribavirin (P>0.2 for each SNP) was observed, suggesting a specific influence of the genetic determinants of 25(OH)D3 serum levels on hepatocarcinogenesis.
Conclusions/Significance: Our data suggest a relatively weak but functionally relevant role for vitamin D in the prevention of HCV-related hepatocarcinogenesis.
It is long known that Kasugamycin inhibits translation of canonical transcripts containing a 5’-UTR with a Shine Dalgarno (SD) motif, but not that of leaderless transcripts. To gain a global overview of the influence of Kasugamycin on translation efficiencies, the changes of the translatome of Escherichia coli induced by a 10 minutes Kasugamycin treatment were quantified. The effect of Kasugamycin differed widely, 102 transcripts were at least twofold more sensitive to Kasugamycin than average, and 137 transcripts were at least twofold more resistant, and there was a more than 100-fold difference between the most resistant and the most sensitive transcript. The 5’-ends of 19 transcripts were determined from treated and untreated cultures, but Kasugamycin resistance did neither correlate with the presence or absence of a SD motif, nor with differences in 5’-UTR lengths or GC content. RNA Structure Logos were generated for the 102 Kasugamycin-sensitive and for the 137 resistant transcripts. For both groups a short Shine Dalgarno (SD) motif was retrieved, but no specific motifs associated with resistance or sensitivity could be found. Notably, this was also true for the region -3 to -1 upstream of the start codon and the presence of an extended SD motif, which had been proposed to result in Kasugamycin resistance. Comparison of the translatome results with the database RegulonDB showed that the transcript with the highest resistance was leaderless, but no further leaderless transcripts were among the resistant transcripts. Unexpectedly, it was found that translational coupling might be a novel feature that is associated with Kasugamycin resistance. Taken together, Kasugamycin has a profound effect on translational efficiencies of E. coli transcripts, but the mechanism of action is different than previously described.
Opportunities to treat infection with hepatitis C virus (HCV) are evolving rapidly. From the introduction of interferon-α monotherapy in 1992 to the approval of telaprevir- and boceprevir-based triple therapies with pegylated interferon-α and ribavirin in 2011, the chances of curing patients infected with HCV genotype 1 have improved from <10% to approximately 70%. Significant further improvements are on the horizon, which may well cure virtually all hepatitis C patients with an all-oral, interferon-free regimen in the very near future. These exciting developments are reviewed in the present article.
Background: To perform a comprehensive study on the relationship between vitamin D metabolism and the response to interferon-α-based therapy of chronic hepatitis C.
Methodology/Principal Findings: Associations between a functionally relevant polymorphism in the gene encoding the vitamin D 1α-hydroxylase (CYP27B1-1260 rs10877012) and the response to treatment with pegylated interferon-α (PEG-IFN-α) and ribavirin were determined in 701 patients with chronic hepatitis C. In addition, associations between serum concentrations of 25-hydroxyvitamin D3 (25[OH]D3) and treatment outcome were analysed. CYP27B1-1260 rs10877012 was found to be an independent predictor of sustained virologic response (SVR) in patients with poor-response IL28B genotypes (15% difference in SVR for rs10877012 genotype AA vs. CC, p = 0.02, OR = 1.52, 95% CI = 1.061–2.188), but not in patients with favourable IL28B genotype. Patients with chronic hepatitis C showed a high prevalence of vitamin D insufficiency (25[OH]D3<20 ng/mL) during all seasons, but 25(OH)D3 serum levels were not associated with treatment outcome.
Conclusions/Significance: Our study suggests a role of bioactive vitamin D (1,25[OH]2D3, calcitriol) in the response to treatment of chronic hepatitis C. However, serum concentration of the calcitriol precursor 25(OH)D3 is not a suitable predictor of treatment outcome.
Die Autoimmunerkrankungen Typ 1 Diabetes mellitus, autoimmune Schilddrüsenerkrankungen und Morbus Addison betreffen zusammen etwa 3% der Bevölkerung. Ihre Genese ist multifaktoriell, also von verschiedenen endo- als auch exogenen Faktoren abhängig. Sie alle tragen gemeinsame prädisponierende genetische Merkmale, das wichtigste unter ihnen ist der Risikogenort der HLA- Gene auf Chromosom sechs, der etwa die Hälfte des genetischen Risikos vermittelt. Für Typ 1 Diabetes mellitus wurde die Insulingenregion als ein weiterer wichtiger Risikogenort postuliert. Im Rahmen dieser Arbeit wurde dieser Zusammenhang in der deutschen Bevölkerung untersucht und nach einer eventuellen Assoziation dieses Genortes mit anderen Autoimmunerkrankungen geforscht. Bei den Untersuchungen in der Insuligenregion wurde der Polymorphismus -2221 MspI (C/T) in der Promotoregion des Insulingens auf seine Assoziation mit der Erkrankung Typ 1 Diabetes mellitus sowie der Erkrankungen Morbus Addison, Morbus Basedow und Hashimoto- Thyreoiditis untersucht. Es wurden auch Patienten genotypisiert, die an einem PGAS- II Syndrom litten, also neben der Erkrankung Typ 1 Diabetes mellitus und/oder Morbus Addison noch zusätzlich an mindestens einer der Erkrankungen Hashimoto- Thyreoiditis oder Morbus Basedow erkrankt waren. Das „C“- Allel der Polymorphismus -2221 MspI (C/T) ist mit den VNTR Klasse IAllelen verknüpft, diese Allele gelten als Risikogenorte für die Entwicklung eines Typ 1 Diabetes mellitus. Das „T“- Allel hingegen steht im Kopplungsungleichgewicht mit den VNTR- Klasse III- Allelen, die vor der Entwicklung eines Typ 1 Diabetes mellitus schützen können. Die von uns untersuchten Typ 1 Diabetiker zeigten das „C“- Allel signifikant häufiger als die Kontrollen (77,2% vs. 49,1%). Es fand sich eine Tendenz zur Bevorzugung des weiblichen Geschlechtes. In den 213 untersuchten Familien transmittierten phänotypisch gesunde Eltern das „C“- Allel signifikant häufiger an ihre erkrankten Kinder als das protektive „T“- Allel. Der Zusammenhang zwischen Polymorphismen der Insulingenregion und den HLA-Risikoallelen wurde bereits in verschiedenen Populationen untersucht und kontrovers diskutiert. Unsere Untersuchungen wiesen bei Kindern mit dem HLAHochrisikohaplotyp DQ2/DQ8 das krankheitsauslösende „C“- Allel signifikant häufiger nach als bei Kindern mit einem niedrigen HLA- Risikohaplotyp. Für Kinder mit einem mittleren HLA- Risiko konnte ebenfalls eine häufigere Transmission des „C“- Allels gezeigt werden. Die von uns untersuchten Patienten mit einer Hashimoto- Thyreoiditis zeigten keine signifikante Assoziation mit dem Polymorphismus, betrachtet man aber nur die erkrankten Frauen in dieser Gruppe, zeigt sich eine borderline- Signifikanz von p=0,0533, was zu der Annahme führt, dass durch eine Erhöhung der Fallzahl durchaus eine Assoziation mit der Erkrankung gezeigt werden könnte. Bei der Gruppe der Addison – Patienten fanden wir eine signifikante Assoziation mit dem Polymorphismus, die aber nicht ganz so deutlich wie bei den Diabetikern war. Hier zeichnet sich eine Bevorzugung des weiblichen Geschlechtes ab. In der Gruppe der PGAS- II Patienten zeigten nur diejenigen Patienten eine Assoziation mit dem Polymorphismus, die an Typ 1 Diabetes mellitus in Kombination mit einer Hashimoto- Thyreoiditis und die an Morbus Addison in Kombination mit Morbus Basedow litten. Bei den anderen Autoimmunendokrinopathien zeigte sich kein Zusammenhang mit der Insulingenregion. Zusammengefasst lässt sich also festhalten, dass der Einfluss der Insulingenregion auf andere Autoimmunerkrankungen neben Typ 1 Diabetes mellitus nicht ausgeschlossen werden kann, ein endgültiger Beweis aber noch zu erbringen ist. Für die Entwicklung von Autoimmunendokrinopathien ist neben der genetischen Komponente aber auch der Einfluss anderer Faktoren von Bedeutung. Eine große Rolle spielt besonders der Einfluss von Vitamin D auf das menschliche Immunsystem. In mehreren Studien wurde gezeigt, dass niedrige Vitamin D Spiegel für die Entwicklung von Typ 1 Diabetes mellitus prädisponieren und die Gabe von 1, 25 (OH)2D3 bei NOD Mäusen die Entstehung die Erkrankung verhindern kann. Zum Erreichen ausreichend hoher Mengen an aktivem Vitamin D wird 25 (OH)D3 nach der glomerulären Filtration in der Niere über die endozytotischen Rezeptoren Megalin und Cubilin wieder rückresorbiert. Bei defektem Megalin- oder Cubilin- Rezeptor konnte ein deutlicher Vitamin D- Verlust nachgewiesen werden. Deshalb wurden im Rahmen dieser Dissertation an Typ 1 Diabetes mellitus erkrankte Patienten auf fünf verschiedenen Polymorphismen des Cubilingens untersucht und nach einem signifikanten Unterschied zwischen dieser Patientengruppe und gesunden Kontrollen gesucht. Die untersuchten Cubilingenpolymorphismen Cub-33, Cub-29, Cub-35 und Cub-68 zeigten keine Assoziation mit der Erkrankung Typ 1 Diabetes mellitus. Beim untersuchten Polymorphismus Cub-65 hingegen zeigten sich zwischen Patienten und Kontrollen signifikante Unterschiede in der Genotypverteilung. Der Genotyp „AA“ kommt bei den untersuchten Patienten zu 26,7% vor, bei den Kontrollen nur zu 5,1%. Allerdings führt der Polymorphismus nicht zur Änderung der Aminosäuresequenz des Cubilins, weshalb seine biologische Bedeutung fraglich ist. Nach heutiger Kenntnis scheint der Polymorphismus also keine Assoziation mit der Erkrankung Typ 1 Diabetes mellitus zu zeigen. Die Bedeutung des Cubilins für die Reabsorption von 25 (OH)D3 bleibt jedoch außer Frage, weshalb weitergehende Untersuchungen mit größeren Patientengruppen auf andere Polymorphismen des Gens einen Zusammenhang zwischen Polymorphismen des Cubilingens und Typ 1 Diabetes mellitus zeigen könnten.
Zusammenfassend steht mit den oben beschriebenen Methoden der interstitiellen HDR-Brachytherapie ein technisch und organisatorisch gut durchführbares Therapiekonzept zur Verfügung, das gut verträglich, wenig invasiv und schnell bei Patienten mit inoperablen lokalen Rektumkarzinomrezidiven wirkt. In Abhängigkeit der verwendeten Technik ist sogar eine ambulante Therapie möglich, ansonsten ist die therapieassoziierte Krankenhausverweildauer kurz. Neben Methoden zur lokalen Tumorkontrolle ist jedoch auch die Weiterentwicklung von systemischen Therapieverfahren entscheidend, da sich tumorbedingte Todesfälle häufig aufgrund von Fernmetastasen ereignen, die sich der lokalen Therapie entziehen.
Overconsumption of carbohydrates and lipids are well known to cause nonalcoholic fatty liver disease (NAFLD), while the role of nutritional protein intake is less clear. In Western diet, meat and other animal products are the main protein source, with varying concentrations of specific amino acids. Whether the amount or composition of protein intake is associated with a higher risk for disease severity has not yet been examined. In this study, we investigated associations of dietary components with histological disease activity by analyzing detailed 14‐day food records in a cohort of 61 patients with biopsy‐proven NAFLD. Furthermore, we used 16S ribosomal RNA gene sequencing to detect associations with different abundances of the gut microbiota with dietary patterns. Patients with definite nonalcoholic steatohepatitis (NAFLD activity score of 5‐8 on liver biopsy) had a significantly higher daily relative intake of protein compared with patients with a NAFLD activity score of 0‐4 (18.0% vs. 15.8% of daily protein‐based calories, P = 0.018). After adjustment for several potentially confounding factors, a higher protein intake (≥17.3% of daily protein‐based calories) remained associated with definite nonalcoholic steatohepatitis, with an odds ratio of 5.09 (95% confidence interval 1.22‐21.25, P = 0.026). This association was driven primarily by serine, glycine, arginine, proline, phenylalanine, and methionine. A higher protein intake correlated with a lower Bacteroides abundance and an altered abundance of several other bacterial taxa. Conclusion: A high protein intake was independently associated with more active and severe histological disease activity in patients with NAFLD. Further studies are needed to investigate the potential harmful role of dietary amino acids on NAFLD, with special attention to meat as their major source.
Neben dem zentralen Benzodiazepin-Rezeptor – einer Untereinheit des GABAA-Rezeptors – gibt es den peripheren Benzodiazepin-Rezeptor (PBR), der als Bestandteil der mitochondrialen Permeabilitäts-modulierenden Poren eine wichtige Rolle bei der Apoptose, Zellproliferation, Steroidsynthese, Immunmodulation, des Porphyrintransports, der Hämsynthese, des Anionentransports und der Regulation der Mitochondrienfunktion spielt. Es gibt zahlreiche endogene und exogene Liganden, welche die Aktivität des PBR modulieren können. Zu den exogenen PBR-Liganden zählen u.a. die Benzodiazepine, Isochinolin- und Pyridinoindol-Derivate, zu den endogenen der Diazepam-Bindungsinhibitor (DBI) und dessen Fragmente sowie Porphyrine und Cholesterin. Man findet den PBR im gesamten Organismus, insbesondere in steroidproduzierenden Geweben wie Nebennierenrinde und Ovarien sowie im Immunsystem, und seine Expression ist stark erhöht bei akutem Stress, bei Entzündungen sowie in Tumorzellen. Bei verschiedenen Karzinomtypen ist der PBR überexprimiert und sein Gehalt korreliert mit der Aggressivität, Progression und Metastasierung des Tumors, sowie einer schlechten Prognose. Untersuchungen mit verschiedenen Mammakarzinom-Zelllinien ergaben, dass zwischen der Proliferationsrate und der Konzentration des PBR ein Zusammenhang besteht und dass in rasch proliferierenden, hormonrezeptornegativen Zelllinien (z.B. BT-20) die Expression der PBR erheblich höher ist als in langsamer wachsenden, hormonrezeptorpositiven Zelllinien (z.B. MCF-7). Der DBI ist ein relativ kurzlebiges Polypeptid, bei dessen proteolytischem Abbau verschiedene biologisch aktive Fragmente entstehen. Ziel der Arbeit war es, den Einfluss von drei Fragmenten des endogenen PBR-Liganden DBI auf den Zellzyklus und die Proliferation der Estrogen- und Progesteronrezeptor-negativen BT20-Mammakarzinomzellen sowie der Estrogen- und Progesteronrezeptor-positiven MCF-7-Mammakarzinomzellen zu untersuchen. Bei den methodisch aufwändigen Experimenten ging es vor allem darum, die isolierten Zellen unter in vitro Bedingungen physiologisch intakt zu erhalten, um den Einfluss der DBI-Fragmente auf die Proliferationsrate unabhängig von störenden äußeren Faktoren untersuchen zu können. Nach Synchronisation der Zellpopulationen in der G0/1-Phase wurden die Zellen mit steigenden Konzentrationen der DBI-Fragmente über verschiedene Zeiträume inkubiert, anschließend wurde die Verteilung der Zellen in den verschiedenen Zyklusphasen durchflusszytometrisch bestimmt. Es konnte gezeigt werden, dass mit allen verwendeten Fragmenten des DBI eine Wachstumshemmung der Karzinomzellen eintrat, wobei anzunehmen ist, dass der Effekt durch die Bindung an den PBR zustande kam. Im Allgemeinen trat diese Wirkung unabhängig von der Hormonsensitivität der verwendeten Zellen auf. Lediglich bei den Zellen in der G2/M-Phase konnte ein Unterschied festgestellt werden, wobei es bei der hormonrezeptorpositiven Zelllinie zu einer Zunahme und bei den hormonrezeptornegativen Mammakarzinomzellen zu einer Abnahme kam. Es zeigte sich, dass zwischen der Wirkung der endogenen PBR-Liganden und der von exogenen Liganden – die in früheren Untersuchungen untersucht worden war - ein auffälliger Unterschied besteht. Unter der Inkubation mit steigenden Konzentrationen der exogenen Liganden kam es zu einem biphasischen Wirkungsverlauf; in sehr niedrigen Konzentrationen kam es zu einer verstärkten Proliferationsrate, während unter hohen Konzentrationen eine Hemmung der Proliferation beobachtet wurde. Möglicherweise ist dieser unerklärliche Effekt der synthetischen Substanzen bereits auf eine zytotoxische Konzentration zurückzuführen. Eine klinische Relevanz der vorliegenden Ergebnisse könnte sich aus dem bereits bei sehr niedrigen Konzentrationen auftretenden wachstumshemmenden Effekt der endogenen DBI-Fragmente auf Tumorzellen ableiten lassen. Als Optionen kämen entweder eine durch Pharmaka induzierte Stimulation der endogenen DBI-Freisetzung oder der Einsatz von länger wirksamen synthetischen Analoga des DBI in Frage. Alternativ könnte auch die Behandlung mit exogenen Liganden des PBR, z.B. mit Benzodiazepinen oder anderen geeigneten synthetischen Substanzen, als therapeutische Möglichkeit entwickelt werden. Allerdings dürfte der bisher beobachtete biphasische Effekt dieser Substanzgruppe, deren proliferationshemmende und apoptosefördernde Wirkung erst in höheren Dosierungen zum Tragen käme, ein erhebliches Problem darstellen. Als weitere therapeutische Möglichkeiten wurde die Nutzung von anti-inflammatorischen Eigenschaften von PBR-Liganden, ihr Einsatz als Chemosensitizer oder als photosensitivierende Agenzien genannt.
Auswirkungen einer Mutation des Adhäsionrezeptors PSGL-1 bei humanen neutrophilen Granulozyten

Der VNTR Polymorphismus des PSGL-1 ist schon seit längerer Zeit Gegenstand wissenschaftlicher Untersuchungen. Der Rezeptor weist je nach Genotyp eine unterschiedliche Länge auf. Dabei ist der Rezeptor mit dem Allel A der längste, das C-Allel weist phänotypisch den kürzesten Rezeptor auf. Der Rezeptor mit dem B-Allel liegt mit der Länge in der Mitte der drei Rezeptoren. In dieser Studie wurde das Roll- und das Adhäsionsverhalten von Leukozyten mit den Genotypen AA bzw. BB mit Hilfe eines Flusskammermodells untersucht. Dieses Verhalten kommt im Rahmen der Bindung zustande, welche P-Selektin und PSGL-1 miteinander eingehen. Das Blut von acht Probanden wurde untersucht, vier Probanden wiesen den Genotyp AA, die anderen vier den Genotyp BB auf. Die Leukozyten wurden aus dem Vollblut isoliert, verdünnt und in das Flusskammermodell eingespeist. Die verdünnte Zellsuspension wurde mittels einer Pumpe über ein Schlauchsystem durch eine Messkammer befördert, welche im Vorfeld mit P-Selektin beschichtet wurde. Dort konnten die Zellen mikroskopisch untersucht und videotechnisch dokumentiert werden. Anschließend konnte die Rollgeschwindigkeit der neutrophilen Granulozyten und die Anzahl der adhärenten Zellen an der Oberfläche durch Computerprogramme ermittelt werden. Als Resultat ergab sich ein signifikanter Unterschied zwischen der Rollgeschwindigkeit der Zellen des Genotyps AA und der Zellen des Genotyps BB. Dabei wiesen die Zellen mit dem B-Allel wegen der geringeren Länge des PSGL-1 eine höhere Rollgeschwindigkeit auf, als die Zellen mit dem AAllel, deren PSGL-1 länger ist. Zudem blieben signifikant weniger Leukozyten mit dem BAllel an der Oberfläche haften als die Leukozyten der anderen Gruppe. Daraus lässt sich schließen, dass die Bindungskraft zwischen PSGL-1 und PSelektin von der Länge des PSGL-1 abhängig ist. Der kürzere Rezeptor (B-Allel des PSGL-1) weist also eine geringere Bindungskraft auf als der längere Rezeptor (A-Allel des PSGL-1). Die Bindung zwischen PSGL-1 und PSelektin lässt sich wahrscheinlich pharmakologisch beeinflussen. Es wurden Antikörper entwickelt, die in klinischen Studien Erfolg versprechende Ansätze bei der Entzündungshemmung gezeigt haben. Die aktuelle Literatur zu klinisch pharmakologischen Behandlungsansätzen wurde in dieser Arbeit diskutiert.
Background: Kidney transplant recipients (KTR) reflect a high-risk population for coronary artery disease (CAD). CAD is the most common cause for morbidity and mortality in this population. However, only few data are available on the favourable revascularization strategy for these patients as they were often excluded from studies and not mentioned in guidelines.
Methods: This retrospective single-centre study includes patients with a history of kidney transplantation undergoing myocardial revascularization for multivessel or left main CAD by either percutaneous coronary intervention (PCI, n = 27 patients) or coronary artery bypass grafting (CABG, n = 24 patients) at University Hospital Frankfurt, Germany, between 2005 and 2015.
Results: In-hospital mortality was higher in the CABG group (20.8% vs. 14.8% PCI group; p = 0.45). In Kaplan-Meier analysis, one-year-survival showed better outcome in the PCI group (85.2% vs. 75%). After four years, outcome was comparable between both strategies (PCI 66.5% vs. CABG 70.8%; log-rank p = 0.94).
Acute kidney injury (AKI), classified by Acute Kidney Injury Network, was observed more frequently after CABG (58.3% vs. 18.5%; p < 0.01). After one year, graft survival was 95.7% in the PCI group and 94.1% in the CABG group. Four year follow-up showed comparable graft survival in both groups (76.8% PCI and 77.0% CABG; p = 0.78).
Conclusion: In this retrospective single-centre study of KTR requiring myocardial revascularization, PCI seems to be superior to CABG with regard to in-hospital mortality, acute kidney injury and one-year-survival. To optimise treatment of these high-risk patients, larger-scaled studies are urgently warranted.
Loss of neuronal stimulation enhances protein breakdown and reduces protein synthesis, causing rapid loss of muscle mass. To elucidate the pathophysiological adaptations that occur in atrophying muscles, we used stable isotope labelling and mass spectrometry to quantify protein expression changes accurately during denervation-induced atrophy after sciatic nerve section in the mouse gastrocnemius muscle. Additionally, mice were fed a stable isotope labelling of amino acids in cell culture (SILAC) diet containing 13C6-lysine for 4, 7 or 11 days to calculate relative levels of protein synthesis in denervated and control muscles. Ubiquitin remnant peptides (K-ε-GG) were profiled by immunoaffinity enrichment to identify potential substrates of the ubiquitin-proteasomal pathway. Of the 4279 skeletal muscle proteins quantified, 850 were differentially expressed significantly within 2 weeks after denervation compared with control muscles. Moreover, pulse labelling identified Lys6 incorporation in 4786 proteins, of which 43 had differential Lys6 incorporation between control and denervated muscle. Enrichment of diglycine remnants identified 2100 endogenous ubiquitination sites and revealed a metabolic and myofibrillar protein diglycine signature, including myosin heavy chains, myomesins and titin, during denervation. Comparative analysis of these proteomic data sets with known atrogenes using a random forest approach identified 92 proteins subject to atrogene-like regulation that have not previously been associated directly with denervation-induced atrophy. Comparison of protein synthesis and proteomic data indicated that upregulation of specific proteins in response to denervation is mainly achieved by protein stabilization. This study provides the first integrated analysis of protein expression, synthesis and ubiquitin signatures during muscular atrophy in a living animal.
Background: CML presenting with a variant Philadelphia translocation, atypical BCR-ABL transcript, additional chromosomal aberrations, and evolving MDS is uncommon and therapeutically challenging. The prognostic significance of these genetic findings is uncertain, even as singular aberrations, with nearly no data on management and outcome when they coexist. MDS evolving during the course of CML may be either treatment-associated or an independently coexisting disease, and is generally considered to have an inferior prognosis. Tyrosine kinase inhibitors (TKI) directed against BCR-ABL are the mainstay of treatment for CML, whereas treatment modalities that may be utilized for MDS and CML include allogeneic stem cell transplant and – at least conceptually – hypomethylating agents.
Case report: Here, we describe the clinical course of such a patient, demonstrating that long-term combined treatment with dasatinib and azacitidine for coexisting CML and MDS is feasible and well tolerated, and may be capable of slowing disease progression. This combination therapy had no deleterious effect on subsequent potentially curative haploidentical bone marrow transplantation.
Conclusions: The different prognostic implications of this unusual case and new therapeutic options in CML are discussed, together with a review of the current literature on CML presenting with different types of genomic aberrations and the coincident development of MDS. Additionally, this case gives an example of long-term combined treatment of tyrosine kinase inhibitors and hypomethylating agents, which could be pioneering in CML treatment.
Cancer is characterized by a remarkable intertumoral, intratumoral, and cellular heterogeneity that might be explained by the cancer stem cell (CSC) and/or the clonal evolution models. CSCs have the ability to generate all different cells of a tumor and to reinitiate the disease after remission. In the clonal evolution model, a consecutive accumulation of mutations starting in a single cell results in competitive growth of subclones with divergent fitness in either a linear or a branching succession. Acute lymphoblastic leukemia (ALL) is a highly malignant cancer of the lymphoid system in the bone marrow with a dismal prognosis after relapse. However, stabile phenotypes and functional data of CSCs in ALL, the so-called leukemia-initiating cells (LICs), are highly controversial and the question remains whether there is evidence for their existence. This review discusses the concepts of CSCs and clonal evolution in respect to LICs mainly in B-ALL and sheds light onto the technical controversies in LIC isolation and evaluation. These aspects are important for the development of strategies to eradicate cells with LIC capacity. Common properties of LICs within different subclones need to be defined for future ALL diagnostics, treatment, and disease monitoring to improve the patients’ outcome in ALL.
B-cell precursor acute lymphoblastic leukemia (BCP-ALL) is an aggressive hematologic malignancy of bone-marrow (BM)-derived lymphoid precursor cells at various stages of differentiation. Although first-line therapy with chemotherapy and—in the case of BCR-ABL1 positive ALL—tyrosine kinase inhibitors is initially highly effective with remission rates of >90%, the overall survival rate in adult patients is 40–50% across all risk groups. Relapse originates from putative leukemia-initiating cells (LICs) that are intrinsically resistant to chemotherapeutic regimens, which may explain the poor long-term prognosis of patients with disease recurrence. Eradication of LICs thus is a principal aim of novel therapeutic approaches. A prerequisite for developing effective LIC-targeted treatments is the ability to identify and clinically monitor LICs in ALL, a goal that has to date been elusive. Temporal information processing in short- and long-term memory of patients with schizophrenia

Cognitive deficits of patients with schizophrenia have been largely recognized as core symptoms of the disorder. One neglected factor that contributes to these deficits is the comprehension of time. In the present study, we assessed temporal information processing and manipulation from short- and long-term memory in 34 patients with chronic schizophrenia and 34 matched healthy controls. On the short-term memory temporal-order reconstruction task, an incidental or intentional learning strategy was deployed. Patients showed worse overall performance than healthy controls. The intentional learning strategy led to dissociable performance improvement in both groups. Whereas healthy controls improved on a performance measure (serial organization), patients improved on an error measure (inappropriate semantic clustering) when using the intentional instead of the incidental learning strategy. On the long-term memory script-generation task, routine and non-routine events of everyday activities (e.g., buying groceries) had to be generated in either chronological or inverted temporal order. Patients were slower than controls at generating events in the chronological routine condition only. They also committed more sequencing and boundary errors in the inverted conditions. The number of irrelevant events was higher in patients in the chronological, non-routine condition. These results suggest that patients with schizophrenia imprecisely access temporal information from short- and long-term memory. In short-term memory, processing of temporal information led to a reduction in errors rather than, as was the case in healthy controls, to an improvement in temporal-order recall. When accessing temporal information from long-term memory, patients were slower and committed more sequencing, boundary, and intrusion errors. Together, these results suggest that time information can be accessed and processed only imprecisely by patients who provide evidence for impaired time comprehension. This could contribute to symptomatic cognitive deficits and strategic inefficiency in schizophrenia.
Anhand der Daten von 2394 Ureterorenoskopien, die innerhalb von 12 Jahren zur Diagnostik und Therapie von Erkrankungen des Harnleiters und des Nierenhohlsystems in der urologischen Klinik des Klinikums Fulda und der urologischen Klinik des St. Elisabethen-Krankenhauses in Frankfurt durchgeführt wurden, wurden in unserer Studie retrospektiv die Effektivität und die Risiken dieses Verfahrens untersucht. Aus den Ergebnisse und der zur Verfügung stehenden Literatur wurden die Gefahren im Detail identifiziert und zur Prävention von Komplikationen jeweils Strategien angeboten. Die Effektivität belief sich auf insgesamt 74% bei der Entfernung von Harnleitersteinen unabhängig von Lage und Größe des Steines und 69% bei der Diagnostik von Neoplasien im Harnleiter. Die größten Erfolge bei niedrigstem Risiko ließen sich bei Steinen im distalen Harnleiterdrittel erreichen (89%), während bei Steinen im mittleren und proximalen Harnleiterdrittel bei höherem operativen Risiko die Erfolgraten bei (80%) lagen. Die Risiken wurden anhand der aufgetretenen Komplikationen beurteilt. Die Gesamtkomplikationsrate unserer Untersuchung beläuft sich auf 18,5% und entspricht der in der zur Verfügung stehenden Literatur. Sie umfasst intraoperative sowie frühe und späte postoperative Komplikationen. Wir haben in der vorliegenden Studie die Ergebnisse und die entstandenen Komplikationen genau analysiert und die Gefahren aufgezeigt und kommen zur Schlussfolgerung: Die Ureterorenoskopie hat sich als effektives und kostengünstiges Verfahren zur Diagnostik und Therapie von Erkrankungen des Harnleiters und des Nierenhohlsystems etabliert. Eine noch kritischere Indikationsstellung, eine sorgfältige Vorbereitung des Patienten und des Operateurs samt OP- Personals, den ausschließlichen Einsatz von Miniscopen und ein bis ins Detail standardisiertes Vorgehen sind geeignet die Komplikationsrate der Ureterorenoskopie in Zukunft deutlich zu senken und die Sicherheit weiter zu erhöhen.
Die Atherosklerose stellt eine der Haupttodesursachen der westlichen Zivilisation dar. Die Karotisbifurkation ist eine der Lokalisationsorte für die Formierung einer atherosklerotischen Läsion, die sich nach Lumeneinengung schliesslich in neurologischen Symptomen bemerkbar machen kann. In einer fortgeschrittenen, symptomatisch gewordenen Karotisplaque kann als charakteristisches Merkmal oftmals ein in der Literatur unter dem Begriff der „Ruptur“ bekannte Oberflächendefekt ausgemacht werden. Es gibt allerdings gehäuft Hinweise über eine Thrombusausbildung auf einer intakten Plaqueoberfläche. Es wird vermutet, dass hier apoptotische Endothelzellen eine entscheidende Rolle bei Veränderungen der Gefässwand spielen, die letzlich für die Thrombusbildung verantwortlich ist. Aus diesen Überlegungen heraus untersuchten wir in der vorliegenden Studie das Ausmaß von Endothelzellapoptosen innerhalb eines Karotisplaque bei symptomatischen und asymptomatischen Patientengruppen. Insgesamt wurden 38 stationäre Patienten mit einer ≥70%-igen ACI-Stenose in die Studie eingeschlossen. Als symptomatisch galten die Patienten, die im Sinne von retinalen oder zerebralen TIA’s oder durch sog. „minor strokes“ in einem Zeitraum von 60 Tagen vor dem Operationstag auffällig geworden waren. Das asymptomatische Patientengut bestand aus den Patienten, bei denen die Karotisstenose als „Zufallsbefund“ aus Routineuntersuchungen in Klinik oder bei niedergelassenen Kollegen entdeckt wurde. Die Plaques wurden “en bloc” exzidiert. In den überwiesenden 33 Fällen wurde die Eversionstechnik durch den Operateur angewendet, bei der das Restlumen unbeschädigt bleibt. Nach Entfernung wurden die Präparate umgehend in 4%-iges Formalin fixiert. Nach Dekalzifizierung erfolgte die transversale Sektionierung in 2mm Scheiben. Jeder 2mm Block wurde in Paraffin eingebettet. Insgesamt wurde dann die Gesamtheit aller 279 Blöcke weiter in 3μm dicke Sektionen geschnitten. Das Vorliegen einer rupturierten Gefässwand wurde als ein intimaler Defekt grösser als 1000μm Breite definiert und für jedes Präparat dokumentiert. Die vitalen Zellen der Endothelschicht wurden durch ein standardisiertes Färbeprotokoll (CD31 Immunhistochemie) sichtbar gemacht. Apoptotische Endothelzellen konnten nach Anwendung der TUNEL-Technik visualisiert werden. Die Färbemethoden wurden jeweils zur Orientierung an angrenzenden Sektionen des Plaques durchgeführt. Die Bilder wurden digitalisiert und bei 40-facher Vergrösserung ausgezählt. Eine Endothelzelle wurde als apoptotisch gewertet, wenn sie jeweils eine positive CD31-Markierung sowie eine positive TUNEL-Färbung in angrenzenden Gewebsschnitten auf sich vereinte. Um das Ausmass der Apoptosen in den Reihen des Endothels zu quantifizieren wurde zunächst die Prozentzahl apoptotischer Endothelzellen pro Plaque Sektion ermittelt und anschliessend durch Berechnung des Medians aller konsekutiven Sektionen die mittlere Prozentzahl apoptotischer Zellen erhoben. Insgesamt war das Auftreten von Apoptose innerhalb des Endothel rar; trotzdem zeigte die symptomatischen Patientengruppe eine signifikant höhere mittlere Prozentsatz apoptotischer Endothelzellen im Vergleich zu dem asymptomatischen Kollektiv. Zwischen den rupturierten und unrupturierten Plaques konnte kein signifikantes Ergebnis erzielt werden, beide Gruppen unterschieden sich kaum. Trotz des möglichen Stellenwerts apoptotischer Endothelzellen bei der fortgeschrittenen Atherosklerose existieren bis heute keine Studien, die das Ausmass derselben in Beziehung zu klinischen Symptomen gebracht haben. Bislang konnte der Nachweis dieser Zellreihe in atherosklerotischen Karotisplaques besonders im poststenotischen Bereich erbracht werden. Veränderungen lokaler Hämodynamik scheinen hier eine Rolle bei der Induktion von Apoptose zu spielen. Da in der hier vorliegenden Arbeit sich der Grad der Stenose kaum unterscheidet müssen alternative Wege bestehen, die Apoptose in atheromatösen Plaques fördert. Apoptotische Endothelien fördern ihrerseits prokoagulatorische Eigenschaften durch vermehrte Expression von thrombogenen Faktoren der Gerinnungskaskade. Das klassische Dogma der Plaqueruptur mit konsekutiver Freilegung des nekrotischen Kerns und Thrombusbildung muss daher erweitert werden, da nicht alle Plaques der vorliegenden Studie diese Oberflächendefekte aufwiesen. Durch das leichte Übergewicht des symptomatischen Kollektivs lassen sich diese Theorien der Apoptose als kritischer Faktor in der Progression atherosklerotischer Herde weiter untermauern, benötigen aber weitere Studien um unter Umständen völlig andere Mechanismen aufzudecken.
Zusammenfassung der 4-Zeilen vs. 16-Zeilen MDCT Untersuchung der Bypässe Der Zweck dieser Studie war es die Bildqualität von aortokoronaren Bypassdarstellungen in 4- und 16-Zeilen Multidetektor-CT mit verschiedenen Bildreformationen zu untersuchen. Materialien und Methoden: 116 Patienten unterzogen sich einer CTUntersuchung nach einer Bypassoperation. Gruppe A (n=58) wurde mit einem 4-Zeiler untersucht; Gruppe B (n=58) wurde mit einem 16-Zeiler untersucht. Verschiedene Bypasstypen wie die LITA zur LAD und venöse Gefäße zur RCA und zur RCX wurden mit in die Studie eingeschlossen. Eine Fünf-Punkte-Lickert-Skala wurde zur Beurteilung der Bildqualität benutzt. Jeder Bypass wurde mitverschiedenen Bildreformationen betrachtet: MIP thin, MPR, VRT. Besonderes Augenmerk wurde auf die Darstellbarkeit der distalen Anastomose gelegt. Die Korrelation zwischen den Untersuchern wurde festgestellt. Resultate: Von 289 untersuchten Bypassgefäßen wurden 279 (96,54%) als funktionsfähig und 10 (3,46%) als nicht funktionsfähig klassifiziert. Bis auf die distalen Anastomosen zeigte der 16-Zeiler signifikant bessere Resultate in allen Segmenten der Bypasses. Vergleiche der Reformationen innerhalb der Gruppen A und B zeigten das mit der MIP thin (p<0,05) und der VRT (p<0,05) bessere Bildqualitäten möglich sind als im Vergleich mit MPR. Schlussfolgerung: Signifikant bessere Darstellbarkeit aller Bypasstypen ist mit dem 16-Zeiler im Vergleich zum 4-Zeiler möglich. Bei der distalen Anastomose scheint kein Unterschied bei der Darstellbarkeit zwischen den beiden 4- bzw. 16- Zeiler Technologien zu bestehen. Zusammenfassung der 4-Zeilen vs. 16-Zeilen MDCT proximale Aortenkonnektoren Untersuchung Der Zweck dieser Studie war es die Bildqualität der proximalen Anastomose von aortokoronaren Bypässen zu untersuchen die mit Hilfe von Nitinolkonnektoren an der Aorta befestigt wurden. Materialien und Methoden: 33 Patienten wurden sofort nach der Bypassoperation mit dem CT untersucht. Die selben Patienten wurden am fünften postoperativen Tag (4Zeilen CT/Gruppe A) und ein Jahr später nochmals (16-Zeiler CT/Gruppe B) untersucht. 23 ACVB auf RCX Bypässe und 27 ACVB auf RCA Bypässe wurden in die Studie eingeschlossen. Jede Anastomose wurde mit verschiedenen Reformationen (MIP,MPR und VRT) ausgewertet. Die Korrelation zwischen den Untersuchern wurde festgestellt. Resultate: Fünf Tage nach der Operation wurden 48 von 50 Bypässe als durchgängig und funktionsfähig klassifiziert. Ein Jahr später sind es noch 42 von 50 Bypässe. In beiden Gruppen konnte der Aortenkonnektor in guter Qualität dargestellt werden: 1.82±1.10 (Gruppe A) und 1.93±1.22 (Gruppe B) für ACVB auf RCA Bypässe, die selben Ergebnisse für ACVB auf RCX Bypässe (MPR). Die Bildqualität auf der Aortenkonnektorseite zeigte keinen signifikanten Unterschied zwischen jeglichen Reformationen beider Gruppen). Innerhalb der Gruppen zeigte die MPR eine bessere Darstellbarkeit als die MIP und VRT (P>0.05). Schlussfolgerung: In der Anwesenheit von metallischen Implantaten, hier im Falle der Nitinolkonnektoren, bietet das CT vertrauenwürdige Daten zur Untersuchung der proximalen Anastomose. Das 16-Zeilen CT bringt keine signifikante Verbesserung der Bildqualität. MPR zeigte die beste Darstellbarkeit innerhalb beider Gruppen.
Congenital sensory deprivation can lead to reorganization of the deprived cortical regions by another sensory system. Such cross-modal reorganization may either compete with or complement the “original“ inputs to the deprived area after sensory restoration and can thus be either adverse or beneficial for sensory restoration. In congenital deafness, a previous inactivation study documented that supranormal visual behavior was mediated by higher-order auditory fields in congenitally deaf cats (CDCs). However, both the auditory responsiveness of “deaf” higher-order fields and interactions between the reorganized and the original sensory input remain unknown. Here, we studied a higher-order auditory field responsible for the supranormal visual function in CDCs, the auditory dorsal zone (DZ). Hearing cats and visual cortical areas served as a control. Using mapping with microelectrode arrays, we demonstrate spatially scattered visual (cross-modal) responsiveness in the DZ, but show that this did not interfere substantially with robust auditory responsiveness elicited through cochlear implants. Visually responsive and auditory-responsive neurons in the deaf auditory cortex formed two distinct populations that did not show bimodal interactions. Therefore, cross-modal plasticity in the deaf higher-order auditory cortex had limited effects on auditory inputs. The moderate number of scattered cross-modally responsive neurons could be the consequence of exuberant connections formed during development that were not pruned postnatally in deaf cats. Although juvenile brain circuits are modified extensively by experience, the main driving input to the cross-modally (visually) reorganized higher-order auditory cortex remained auditory in congenital deafness.
Background: Glaucoma is a neurodegenerative disease, leading to thinning of the retinal nerve fibre layer (RNFL). The exact influence of ocular, cardiovascular, morphometric, lifestyle and cognitive factors on RNFL thickness (RNFLT) is unknown and was analysed in a subgroup of the Gutenberg Health Study (GHS).
Methods: Global peripapillary RNFLT was measured in 3224 eyes of 1973 subjects (49% female) using spectral-domain optical coherence tomography (SD-OCT). The association of age, sex, ocular, cardiovascular, morphometric, lifestyle and cognitive factors on RNFLT was analysed using Pearson correlation coefficient and fitting a linear mixed model.
Results: In the univariable analysis highest correlations were found for axial length (r = -0.27), spherical equivalent (r = 0.24), and glaucoma (r = -0.15) (p<0.0001, respectively). Other significant correlations with RNFLT were found for age, sex, intraocular pressure, systemic hypertension and systolic blood pressure, previous eye surgery, cholesterol, homocysteine, history of coronary artery disease, history of myocardial infarction, apnoea, diabetes and alcohol intake, p<0.05, respectively. Body length, body weight, BMI, diastolic blood pressure, blood glucose, HbA1c, history of apoplexy, cognitive function, peripheral artery disease, tinnitus, migraine, nicotine intake, central corneal thickness, and pseudophakia were not significantly correlated with RNFLT. The regression model revealed a significant relationship between RNFLT and age in decades (p<0.02), spherical equivalent (p<0.0001), axial length (p<0.0001), glaucoma (p<0.0001), tinnitus (p = 0.04), apnoea (p = 0.047), homocysteine (p = 0.05) and alcohol intake >10g/d for women and >20g/d for men (p = 0.02). Glaucoma, apnoea, higher homocysteine, higher alcohol intake and higher axial length as well as age were related to decreased RNFLT while higher spherical equivalent or history for tinnitus were related to thicker RNFL.
Conclusion: RNFLT is related to age, ocular parameters and lifestyle factors. Considering these parameters in normative databases could improve the evaluation of peripapillary RNFLT. It is necessary to evaluate if a reduction of alcohol intake as well as the therapy of apnea or high homocysteine levels could positively influence RNFLT.

Im Rahmen der Professionalisierung der medizinischen Aus-, Fort- und Weiterbildung ist eine pädagogisch-didaktische Ausbildung der Lehrenden unumgänglich. Um Orientierung und Argumentationshilfe zu geben, werden in einer dreiteiligen Artikelfolge Stellenwert der Medizindidaktik, Anforderungsprofil der Angebote und Konzepte zur Implementierung und zur Erfolgsmessung für den deutschsprachigen Raum beleuchtet. In Teil II beleuchten wir den Ausbildungsbedarf und erstellen ein Anforderungsprofil für ein strukturiertes systematisches Qualifizierungsangebot. Der Hauptbedarf besteht in der Qualifizierung der Lehrenden, die den täglichen Unterricht durchführen. Insbesondere sie brauchen das Handwerkszeug, das ihnen erlaubt, ihre verschiedenen Lehraufgaben effizienter zu bewältigen. Seitens der Fakultät und der Studierenden bestehen Ansprüche an qualifizierte Lehrende; ebenso bestehen Forderungen seitens der Lehrenden an adäquat qualifizierende Kurse. Zur Umsetzung dieser berechtigten Ansprüche sind modular aufgebaute Programme nötig, die eine zumindest national qualitativ und quantitativ vergleichbare Ausbildung gewährleisten. Anforderungen an derartige Kurse sind bereits definiert und lokal zum Beispiel in Baden-Württemberg und zum Teil auch in Nordrhein-Westfalen umgesetzt. Es gilt nun, diese auf breiter Basis in die Praxis umzusetzen.

In Teil I geben wir eine Bestandsaufnahme zur Medizindidaktik. Aktuell gibt es bundesweit ein breit gefächertes Qualifizierungsangebot. Es reicht von einfachen unstrukturierten Kurzfortbildungen wie zum Beispiel Vorträgen und Seminaren, die inhaltlich, formal und qualitativ eine große Beliebigkeit zeigen, bis hin zu umfassenden mehrjährigen (Aufbau-)Studiengängen mit "Master-Degree". Im internationalen Vergleich fehlt in Deutschland ein allgemein verbindliches "Basis-Programm", das die täglich Lehrenden systematisch auf ihre Ausbildungsaufgaben vorbereitet. Dies ist bisher nur lokal umgesetzt wie zum Beispiel in Baden-Württemberg mit dem ministeriell zertifizierten Programm der Medizindidaktischen Qualifikation I und II. Vergleichbares ist in Nordrhein-Westfalen und Bayern im Aufbau.
Hintergrund: Die Behandlung kindlicher Linsentrübungen stellt nach wie vor eine Herausforderung für die behandelnden Augenärzte dar. Im Bezug auf die Nachstarprävention ist man sich einig, dass das Belassen der intakten Linsenhinterkapsel bei Patienten unterhalb des sechsten Lebensjahres mit einem sehr hohen Nachstarrisiko behaftet ist. Aus diesem Grund haben sich einige Therapieformen mit primärer hinterer Linsenkapseleröffnung etabliert. Die beiden Nachstarpräventionstechniken bei Kindern, vordere Vitrektomie und Knopflochimplantation der Kunstlinse ohne Vitrektomie, wurden in einigen internationalen Studien bereits untersucht. Aus Deutschland stammende Untersuchungsergebnisse aus der kindlichen Kataraktchirurgie, mit einem Beobachtungszeitraum über mehrere Jahre, liegen in der Literatur noch nicht vor.
Zielsetzung. Diese Pilotstudie untersucht den therapeutischen Effekt des RelaxBogens in Bezug auf Bruxismus und Symptome einer kraniomandibulären Dysfunktion (CMD).
Probanden und Methoden. Untersucht wurde eine Gruppe von 10 Probanden. Auswahlkriterien waren ein vorliegender Bruxismus und erste Symptome einer CMD. Dies wurde durch die Anwendung einer Brux-Checker®-Folie für 2 Nächte und eine Schmerzanamnese verifiziert. Eingangs wurden neben einer ausführlichen zahnärztlichen Anamnese folgende CMDParameter erhoben: SL-NRS-Fragebogen sowie Palpation von 42 Muskeln im Kopf-HalsNacken-Bereich und der 6 Austrittspunkte des N. trigeminus. Nach der Voruntersuchung wurde der RelaxBogen mindestens 10 Wochen getragen. Danach wurden die genannten Parameter erneut erhoben und miteinander verglichen.
Ergebnisse. Die Vor- und Nachuntersuchungen zeigten starke Tendenzen der Reduktion sowohl der allgemeinen Symptomatik als auch der Schmerzempfindung im Kieferbereich. Nach dem Tragen des RelaxBogens konnte eine eindeutige Reduktion des durch Palpation ausgelösten, muskulären Schmerzes beobachtet werden. Ebenfalls deutlich positive Tendenzen ließen sich bei der Verbesserung des allgemeinen Wohlbefindens finden. Die Ergebnisse legen nahe, dass bei einer größeren Stichprobe eine statistische Signifikanz zu erwarten ist.
Schlussfolgerung. Der RelaxBogen führt nach den Ergebnissen dieser Pilotstudie bei Patienten, die unter Bruxismus- und CMD-Symptomen leiden, zu einer deutlichen Reduktion der Schmerzwahrnehmung sowie einer eindeutigen Steigerung des Wohlbefindens. Neben der Verringerung des Schmerzempfindens und einem reduzierten Spannungsgefühl in den großen Kiefermuskeln konnten auch positive Tendenzen im Bereich der Hals-, Nacken- und Schultermuskulatur festgestellt werden.
Macroautophagy requires membrane trafficking and remodelling to form the autophagosome and deliver its contents to lysosomes for degradation. We have previously identified the TBC domain‐containing protein, TBC1D14, as a negative regulator of autophagy that controls delivery of membranes from RAB11‐positive recycling endosomes to forming autophagosomes. In this study, we identify the TRAPP complex, a multi‐subunit tethering complex and GEF for RAB1, as an interactor of TBC1D14. TBC1D14 binds to the TRAPP complex via an N‐terminal 103 amino acid region, and overexpression of this region inhibits both autophagy and secretory traffic. TRAPPC8, the mammalian orthologue of a yeast autophagy‐specific TRAPP subunit, forms part of a mammalian TRAPPIII‐like complex and both this complex and TBC1D14 are needed for RAB1 activation. TRAPPC8 modulates autophagy and secretory trafficking and is required for TBC1D14 to bind TRAPPIII. Importantly, TBC1D14 and TRAPPIII regulate ATG9 trafficking independently of ULK1. We propose a model whereby TBC1D14 and TRAPPIII regulate a constitutive trafficking step from peripheral recycling endosomes to the early Golgi, maintaining the cycling pool of ATG9 required for initiation of autophagy.
Genetic generalised epilepsy (GGE) is the most common form of genetic epilepsy, accounting for 20% of all epilepsies. Genomic copy number variations (CNVs) constitute important genetic risk factors of common GGE syndromes. In our present genome-wide burden analysis, large (≥ 400 kb) and rare (< 1%) autosomal microdeletions with high calling confidence (≥ 200 markers) were assessed by the Affymetrix SNP 6.0 array in European case-control cohorts of 1,366 GGE patients and 5,234 ancestry-matched controls. We aimed to: 1) assess the microdeletion burden in common GGE syndromes, 2) estimate the relative contribution of recurrent microdeletions at genomic rearrangement hotspots and non-recurrent microdeletions, and 3) identify potential candidate genes for GGE. We found a significant excess of microdeletions in 7.3% of GGE patients compared to 4.0% in controls (P = 1.8 x 10-7; OR = 1.9). Recurrent microdeletions at seven known genomic hotspots accounted for 36.9% of all microdeletions identified in the GGE cohort and showed a 7.5-fold increased burden (P = 2.6 x 10-17) relative to controls. Microdeletions affecting either a gene previously implicated in neurodevelopmental disorders (P = 8.0 x 10-18, OR = 4.6) or an evolutionarily conserved brain-expressed gene related to autism spectrum disorder (P = 1.3 x 10-12, OR = 4.1) were significantly enriched in the GGE patients. Microdeletions found only in GGE patients harboured a high proportion of genes previously associated with epilepsy and neuropsychiatric disorders (NRXN1, RBFOX1, PCDH7, KCNA2, EPM2A, RORB, PLCB1). Our results demonstrate that the significantly increased burden of large and rare microdeletions in GGE patients is largely confined to recurrent hotspot microdeletions and microdeletions affecting neurodevelopmental genes, suggesting a strong impact of fundamental neurodevelopmental processes in the pathogenesis of common GGE syndromes.
Die Magnetresonanzspektroskopie (MRS) ist eine nicht-invasive Methode zur Quantifizierung metabolischer Änderungen des Gehirnstoffwechsels. Einige dieser Metaboliten wie N-Acetyl-Aspartat (tNAA), Myo-Insoitol (mI), Kreatin (tCr) und Cholin (Cho) können auch als Marker für neurodegenerative Erkrankungen (z.B. Morbus Alzheimer) dienen. Bislang haben nur wenige Studien MR-spektroskopische Veränderungen bei Patienten mit der Diagnose einer „leichten kognitiven Beeinträchtigung“ (Mild Cognitive Impairment = MCI) untersucht. Entsprechend sollten in dieser Untersuchung MR-spektroskopische Veränderungen bei MCI-Patienten erfasst und in Abhängigkeit vom klinischen Verlauf in einer Längsschnittstudie analysiert werden. Fünfzehn MCI-Probanden und zwölf gesunde ältere Kontrollpersonen wurden jeweils zweimal innerhalb eines Zeitraumes von ca. 40 Monaten klinisch, neuropsychologisch sowie MR-spektroskopisch untersucht. Die MR-Spektroskopie wurde als Einzelvolumenspektroskopie (SVS) mit kurzer Echozeit (TE = 30 ms) durchgeführt. Als VOI wurde ein Bereich im Parietallappen ausgewählt, der den Gyrus cinguli mit einschloss. Der Anteil an CSF wurde aus der Dekonvolution der T2-Dephasierung des Wassersignals ermittelt. Die Auswertung der Spektren erfolgte mit dem Programm LCModel, das eine Analyse des Spektrums mittels einer Linearkombination von Modellspektren durchführt. Die ermittelten Werte für die Signalintensitäten wurden in absolute Metabolitenkonzentrationen (mmol/l) unter Berücksichtigung entsprechender Korrekturen umgerechnet. Der Mittelwert des MMSE-Scores der MCI-Patienten lag zum Zeitpunkt der Erstuntersuchung bei 26,4 (Bereich 20 - 30) und fiel im Verlauf auf 23,3 (Bereich 14 - 28) ab. Sechs der MCI-Patienten entwickelten im zeitlichen Intervall ein dementielles Syndrom (MCIKonverter). Alle Kontrollpersonen hatten einen MMSE-Score von mehr als 29 in beiden Untersuchungen. Bei den MRS-Untersuchungen wurde bei Erstuntersuchung ein signifikanter Unterschied für den CSF-Anteil des Messvolumens im Vergleich der Patientengruppe und der Probanden festgestellt. Für die MR-spektroskopisch erhobenen Metabolitenkonzentrationen ergaben sich bei der Erstuntersuchung keine signifikanten Unterschiede. Bei der zweiten Untersuchung zeigte sich dagegen eine signifikante Erniedrigung der Konzentrationen von tCr und tNAA der Gruppe der MCIKonverter im Vergleich zu den entsprechenden Konzentrationen der anderen Gruppen. In der MCI-Gruppe war eine Abnahme der tNAA-Konzentgration signifikant mit einer Abnahme der kognitiven Leistung korreliert. Dieser Effekt war im Wesentlichen auf metabolische Änderungen in der Gruppe der MCIKonverter zurückzuführen. Für die Metaboliten Cho und mI konnten keine signifikanten Unterschiede zwischen den einzelnen Gruppen festgestellt werden. Diese Pilotstudie zeigt, dass MR-spektroskopisch gemessene Metaboliten (insbesondere tNAA) bei der MCI als Surrogatmarker eines (in Richtung Demenz) fortschreitenden neurodegenerativen Krankheitsprozesses eingesetzt werden können. Die Ergebnisse der vorliegenden Studie sind mit anderen Publikationen (Ackl et al., 2005; Adalsteinsson et al, 2000; Chao et al, 2005; Kantarci et al., 2000) in Einklang zu bringen. Da auch die tCr-Konzentration im Verlauf Schwankungen unterliegt, wird klar, dass eine Quantifizierung in absoluten Konzentrationen einem relativen Vergleich von tNAA zu tCr vorzuziehen ist.
Since our knowledge on structure and function of messenger RNA (mRNA) has expanded from merely being an intermediate molecule between DNA and proteins to the notion that RNA is a dynamic gene regulator that can be modified and edited, RNA has become a focus of interest into developing novel therapeutic schemes. Therapeutic modulation of RNA molecules by DNA- and RNA-based therapies has broadened the scope of therapeutic targets in infectious diseases, cancer, neurodegenerative diseases and most recently in cardiovascular diseases as well. Currently, antisense oligonucleotides (ASO), small interfering RNAs (siRNAs), and microRNAs are the most widely applied therapeutic strategies to target RNA molecules and regulate gene expression and protein production. However, a number of barriers have to be overcome including instability, inadequate binding affinity and delivery to the tissues, immunogenicity, and off-target toxicity in order for these agents to evolve into efficient drugs. As cardiovascular diseases remain the leading cause of mortality worldwide, a large number of clinical trials are under development investigating the safety and efficacy of RNA therapeutics in clinical conditions such as familial hypercholesterolemia, diabetes mellitus, hypertriglyceridemia, cardiac amyloidosis, and atrial fibrillation. In this review, we summarize the clinical trials of RNA-targeting therapies in cardiovascular disease and critically discuss the advances, the outcomes, the limitations and the future directions of RNA therapeutics in precision transcriptomic medicine.
Expansions of the polyglutamine (polyQ) domain (≥34) in Ataxin-2 (ATXN2) are the primary cause of spinocerebellar ataxia type 2 (SCA2). Recent studies reported that intermediate-length (27–33) expansions increase the risk of Amyotrophic Lateral Sclerosis (ALS) in 1–4% of cases in diverse populations. This study investigates the Turkish population with respect to ALS risk, genotyping 158 sporadic, 78 familial patients and 420 neurologically healthy controls. We re-assessed the effect of ATXN2 expansions and extended the analysis for the first time to cover the ATXN2 locus with 18 Single Nucleotide Polymorphisms (SNPs) and their haplotypes. In accordance with other studies, our results confirmed that 31–32 polyQ repeats in the ATXN2 gene are associated with risk of developing ALS in 1.7% of the Turkish ALS cohort (p = 0.0172). Additionally, a significant association of a 136 kb haplotype block across the ATXN2 and SH2B3 genes was found in 19.4% of a subset of our ALS cohort and in 10.1% of the controls (p = 0.0057, OR: 2.23). ATXN2 and SH2B3 encode proteins that both interact with growth receptor tyrosine kinases. Our novel observations suggest that genotyping of SNPs at this locus may be useful for the study of ALS risk in a high percentage of individuals and that ATXN2 and SH2B3 variants may interact in modulating the disease pathway.
Parkinson's disease (PD) is a frequent neurodegenerative process in old age. Accumulation and aggregation of the lipid-binding SNARE complex component α-synuclein (SNCA) underlies this vulnerability and defines stages of disease progression. Determinants of SNCA levels and mechanisms of SNCA neurotoxicity have been intensely investigated. In view of the physiological roles of SNCA in blood to modulate vesicle release, we studied blood samples from a new large pedigree with SNCA gene duplication (PARK4 mutation) to identify effects of SNCA gain of function as potential disease biomarkers. Downregulation of complexin 1 (CPLX1) mRNA was correlated with genotype, but the expression of other Parkinson's disease genes was not. In global RNA-seq profiling of blood from presymptomatic PARK4 indviduals, bioinformatics detected significant upregulations for platelet activation, hemostasis, lipoproteins, endocytosis, lysosome, cytokine, Toll-like receptor signaling and extracellular pathways. In PARK4 platelets, stimulus-triggered degranulation was impaired. Strong SPP1, GZMH and PLTP mRNA upregulations were validated in PARK4. When analysing individuals with rapid eye movement sleep behavior disorder, the most specific known prodromal stage of general PD, only blood CPLX1 levels were altered. Validation experiments confirmed an inverse mutual regulation of SNCA and CPLX1 mRNA levels. In the 3′-UTR of the CPLX1 gene we identified a single nucleotide polymorphism that is significantly associated with PD risk. In summary, our data define CPLX1 as a PD risk factor and provide functional insights into the role and regulation of blood SNCA levels. The new blood biomarkers of PARK4 in this Turkish family might become useful for PD prediction.
Verschiedene Studien haben sich mit dem Spontanverlauf intrakranieller Aneurysmen beschäftigt mit dem Ziel, Risikofaktoren für eine Ruptur für die Nutzen-Risiko-Abwägung der primärpräventiven Aneurysmabehandlung zu identifizieren. Beachtung fand dabei in erster Linie die Größe und Lokalisation der Aneurysmen. Die Analyse der Aneurysmamorphologie spielte bei den Untersuchungen bisher eine untergeordnete Rolle, war sie doch mit der digitalen Subtraktionsangiographie nur unzureichend zu beurteilen. Durch die Entwicklung der 3D-Rotationsangiographie Ende der neunziger Jahre wurde die Form von Aneurysmen detailgenau erfaßbar. Im klinischen Alltag wird Aneurysmen mit unregelmäßiger Morphologie von den beurteilenden Neuroradiologen und Neurochirurgen ein erhöhtes Rupturrisiko zugeschrieben. Mit dem Ziel diese subjektive Einschätzung mittels einer systematische Formanalyse zu überprüfen, untersuchten wir im Rahmen einer prospektiven Pilotstudie die Form rupturierter (46) und nichtrupturierter (45) intrakranieller Aneurysmen an 70 konsekutiven Patienten. Neben der subjektiven Beurteilung durch zwei erfahrene Neuroradiologen unternahmen wir den Versuch, meßbare morphologische Daten zu erheben. Dazu verwendeten wir ein digitales, auf der Fourier-Analyse basierendes Bildanalysesystem. Die Darstellung der Aneurysmen erfolgte mit Hilfe der 3D-Rotationsangiographie. In der 3D-Darstellung fand sich eine Unregelmäßigkeit der Oberfläche bei 78% der rupturierten und bei 42% der nichtrupturierten Aneurysmen (p<0,05). Die quantitative Formanalyse zeigte bei rupturierten Aneurysmen einen kleineren Rundheitsfaktor (p<0,02) und höhere Werte der Fourieramplituden 2, 3 und 7 (p<0,025) als bei nichtrupturierten Aneurysmen. Diese Kriterien deuten auf eine unregelmäßigere Form rupturierter Aneurysmen hin. Unsere Formanalyse ergab signifikante Unterschiede zwischen rupturierten und nichtrupturierten Aneurysmen. Die Fourier-Analyse erwies sich dabei als hilfreiche Methode, um diese Formunterschiede quantitativ zu dokumentieren. Die Ergebnisse dieser Pilotstudie müssen jedoch anhand von Untersuchungen mit größerer Fallzahl sowie einer multiplanaren quantitativen Formanalyse überprüft werden.
Our study is the first to objectively assess sleep and sleep-related respiration in orchestra musicians. We hypothesized low sleep quality due to high work demands and irregular work-sleep schedules, and a better respiration for wind instrument (WI) players than string instrument (SI) players due to habitual upper airway muscles training. We recorded overnight polysomnography with 29 professional orchestra musicians (21 men, 14 WI/ 15 SI). The musicians presented a sleep efficiency of 88% (IQR 82–92%) with WI having a significant higher sleep efficiency than SI (89%, 85–93% vs. 85%, 74–89%; p = 0.029). The group had a total sleep time around 6 hours (377min, 340-421min) with signs of increased NREM 1 (light sleep) and decreased REM (dream sleep). The musicians displayed an apnea-hypopnea-index of 2.1events/hour (0.7–5.5) and an oxygen saturation of 98% (97–100%). While SI player exhibited declining sleep-related respiration with age (breathing events: r = 0.774, p = 0.001, oxygen: r = -0.647, p = 0.009), WI player showed improved respiration with age (breathing events: r = -0.548, p = 0.043; oxygen: r = 0.610, p = 0.020). Our study is the first objective investigation of sleep pattern and respiration during sleep with overnight polysomnography in professional orchestra musicians. While sleep and respiration were unexpectedly good, our results revealed possible signs of sleep deprivation and an interesting age-related pattern on respiration depending on instrument. While sample size was small and results modest, these findings present first objective evidence towards the assumption that habitual playing of a WI–and training of the upper airway muscles–may have a protective effect on respiration.
Background: High baseline levels of IP-10 predict a slower first phase decline in HCV RNA and a poor outcome following interferon/ribavirin therapy in patients with chronic hepatitis C. Several recent studies report that single nucleotide polymorphisms (SNPs) adjacent to IL28B predict spontaneous resolution of HCV infection and outcome of treatment among HCV genotype 1 infected patients. Methods and Findings: In the present study, we correlated the occurrence of variants at three such SNPs (rs12979860, rs12980275, and rs8099917) with pretreatment plasma IP-10 and HCV RNA throughout therapy within a phase III treatment trial (HCV-DITTO) involving 253 Caucasian patients. The favorable SNP variants (CC, AA, and TT, respectively) were associated with lower baseline IP-10 (P = 0.02, P = 0.01, P = 0.04) and were less common among HCV genotype 1 infected patients than genotype 2/3 (P<0.0001, P<0.0001, and P = 0.01). Patients carrying favorable SNP genotypes had higher baseline viral load than those carrying unfavorable variants (P = 0.0013, P = 0.029, P = 0.0004 respectively). Among HCV genotype 1 infected carriers of the favorable C, A, or T alleles, IP-10 below 150 pg/mL significantly predicted a more pronounced reduction of HCV RNA from day 0 to 4 (first phase decline), which translated into increased rates of RVR (62%, 53%, and 39%) and SVR (85%, 76%, and 75% respectively) among homozygous carriers with baseline IP-10 below 150 pg/mL. In multivariate analyses of genotype 1-infected patients, baseline IP-10 and C genotype at rs12979860 independently predicted the first phase viral decline and RVR, which in turn independently predicted SVR. Conclusions: Concomitant assessment of pretreatment IP-10 and IL28B-related SNPs augments the prediction of the first phase decline in HCV RNA, RVR, and final therapeutic outcome.
In dieser Arbeit wurde im Rahmen einer großen prospektiven, internationalen, multizentrischen, doppelblinden und randomisierten Studie an einer Subgruppe von 44 Patienten mit elektiver Hüftgelenkersatzoperation die Plättchenadhäsivität an silikonisiertem Glas untersucht. Ziel der Gesamtstudie war es, die Effiezienz von drei unterschiedlichen Hirudin-Dosen im Vergleich zu der üblichen Behandlung mit unfraktioniertem Heparin in der antithrombotischen Wirkung zu prüfen. 12 Patienten erhielten 2 x 10 mg, 10 Patienten 2 x 15 mg, 11 Patienten 2 x 20mg Hirudin und 11 Patienten 3 x 5000 IE unfraktioniertes Heparin. Bei allen Patienten wurde am 10. postoperativen Tag eine Phlebographie vorgenommen. Bei 6 Patienten stellte man phlebografisch eine Beinvenenthrombose ohne klinische Symptome fest. Der Plättchenadhäsivitätindex stieg unspezifisch in allen Gruppen am 1.postoperativen Tag an und fiel am 3./4. postoperativen Tag wieder mehr oder weniger gleich stark ab. Präoperativ sowie am 1. und 3./4. postoperativen Tag wurden zusätzlich folgende Parameter bestimmt: aPTT, Heptest, Fibrinogen und F VIII assoziertes Antigen. APTT stieg in beiden Gruppen an. Fibrinogen und F VIII stiegen postoperativ deutlich an. Da aber dieser Anstieg sowohl bei der Thrombosegruppe als auch bei der Nicht-Thrombosegruppe auftrat, hatten auch diese Werte keinen prädiktiven Wert. Der Heptest stieg minimal in beiden Gruppen. Dieser Test ist auch verwendbar zur Überwachung einer Behandlung mit Thrombinhemmern.
INTRODUCTION: Medical societies have developed guidelines for the detection, treatment and control of hypertension (HTN). Our analysis assessed the extent to which such guidelines were implemented in Germany in 2003 and 2001.
METHODS: Using standardized clinical diagnostic and treatment appraisal forms, blood pressure levels and patient questionnaires for 55,518 participants from the cross-sectional Targets and Essential Data for Commitment of Treatment (DETECT) study (2003) were analyzed. Physician's diagnosis of hypertension (HTN(doc)) was defined as coding hypertension in the clinical appraisal questionnaire. Alternative definitions used were physician's diagnosis or the patient's self-reported diagnosis of hypertension (HTN(doc,pat)), physician's or patient's self-reported diagnosis or a BP measurement with a systolic BP≥140 mmHg and/or a diastolic BP≥90 (HTN(doc,pat,bp)) and diagnosis according to the National Health and Nutrition Examination Survey (HTN(NHANES)). The results were compared with the similar German HYDRA study to examine whether changes had occurred in diagnosis, treatment and adequate blood pressure control (BP below 140/90 mmHg) since 2001. Factors associated with pharmacotherapy and control were determined.
RESULTS: The overall prevalence rate for hypertension was 35.5% according to HTN(doc) and 56.0% according to NHANES criteria. Among those defined by NHANES criteria, treatment and control rates were 56.0% and 20.3% in 2003, and these rates had improved from 55.3% and 18.0% in 2001. Significant predictors of receiving antihypertensive medication were: increasing age, female sex, obesity, previous myocardial infarction and the prevalence of comorbid conditions such as coronary heart disease (CHD), hyperlipidemia and diabetes mellitus (DM). Significant positive predictors of adequate blood pressure control were CHD and antihypertensive medication. Inadequate control was associated with increasing age, male sex and obesity.
CONCLUSIONS: Rates of treated and controlled hypertension according to NHANES criteria in DETECT remained low between 2001 and 2003, although there was some minor improvement.
In ischemic vascular diseases, leukocyte recruitment and polarization are crucial for revascularization and tissue repair. We investigated the role of vasodilator-stimulated phosphoprotein (VASP) in vascular repair. After hindlimb ischemia induction, blood flow recovery, angiogenesis, arteriogenesis, and leukocyte infiltration into ischemic muscles in VASP−/− mice were accelerated. VASP deficiency also elevated the polarization of the macrophages through increased signal transducer and activator of transcription (STAT) signaling, which augmented the release of chemokines, cytokines, and growth factors to promote leukocyte recruitment and vascular repair. Importantly, VASP deletion in bone marrow–derived cells was sufficient to mimic the increased blood flow recovery of global VASP−/− mice. In chemotaxis experiments, VASP−/− neutrophils/monocytes were significantly more responsive to M1-related chemokines than wild-type controls. Mechanistically, VASP formed complexes with the chemokine receptor CCR2 and β-arrestin-2, and CCR2 receptor internalization was significantly reduced in VASP−/− leukocytes. Our data indicate that VASP is a major regulator of leukocyte recruitment and polarization in postischemic revascularization and support a novel role of VASP in chemokine receptor trafficking.
Evidence about distribution patterns of brain metastases with regard to breast cancer subtypes and its influence on the prognosis of patients is insufficient. Clinical data, cranial computed tomography (CT) and magnetic resonance imaging (MRI) scans of 300 breast cancer patients with brain metastases (BMs) were collected retrospectively in four centers participating in the Brain Metastases in Breast Cancer Registry (BMBC) in Germany. Patients with positive estrogen (ER), progesterone (PR), or human epidermal growth factor receptor 2 (HER2) statuses, had a significantly lower number of BMs at diagnosis. Concerning the treatment mode, HER2-positive patients treated with trastuzumab before the diagnosis of BMs showed a lower number of intracranial metastases (p < 0.001). Patients with a HER2-positive tumor-subtype developed cerebellar metastases more often compared with HER2-negative patients (59.8% vs. 44.5%, p = 0.021), whereas patients with triple-negative primary tumors had leptomeningeal disease more often (31.4% vs. 18.3%, p = 0.038). The localization of Brain metastases (BMs) was associated with prognosis: patients with leptomeningeal disease had shorter survival compared with patients without signs of leptomeningeal disease (median survival 3 vs. 5 months, p = 0.025). A shorter survival could also be observed in the patients with metastases in the occipital lobe (median survival 3 vs. 5 months, p = 0.012). Characteristics and clinical outcome of breast cancer patients with asymptomatic brain metastases

Simple Summary: The prognosis for patients with breast cancer that has spread to the brain is poor, and survival for these women hasn’t improved over the last few decades. We do not currently test for asymptomatic brain metastases in breast cancer patients, although this does happen in some other types of cancer. In this study we wanted to find out more about breast cancer that has spread to the brain and in particular to see whether there might be any advantage to spotting brain metastases before the development of neurological symptoms. Overall, our results suggest that women could be better off if their brain metastases are diagnosed before they begin to cause symptoms. We now need to carry out a clinical trial to see what happens if we screen high-risk breast cancer patients for brain metastases. This will verify whether doing so could increase survival, symptom control or quality of life.
Abstract: Background: Brain metastases (BM) have become a major challenge in patients with metastatic breast cancer. Methods: The aim of this analysis was to characterize patients with asymptomatic BM (n = 580) in the overall cohort of 2589 patients with BM from our Brain Metastases in Breast Cancer Network Germany (BMBC) registry. Results: Compared to symptomatic patients, asymptomatic patients were slightly younger at diagnosis (median age: 55.5 vs. 57.0 years, p = 0.01), had a better performance status at diagnosis (Karnofsky index 80–100%: 68.4% vs. 57%, p < 0.001), a lower number of BM (>1 BM: 56% vs. 70%, p = 0.027), and a slightly smaller diameter of BM (median: 1.5 vs. 2.2 cm, p < 0.001). Asymptomatic patients were more likely to have extracranial metastases (86.7% vs. 81.5%, p = 0.003) but were less likely to have leptomeningeal metastasis (6.3% vs. 10.9%, p < 0.001). Asymptomatic patients underwent less intensive BM therapy but had a longer median overall survival (statistically significant for a cohort of HER2-positive patients) compared to symptomatic patients (10.4 vs. 6.9 months, p < 0.001). Conclusions: These analyses show a trend that asymptomatic patients have less severe metastatic brain disease and despite less intensive local BM therapy still have a better outcome (statistically significant for a cohort of HER2-positive patients) than patients who present with symptomatic BM, although a lead time bias of the earlier diagnosis cannot be ruled out. Our analysis is of clinical relevance in the context of potential trials examining the benefit of early detection and treatment of BM.
Background: The incidence of central nervous system (CNS) metastases in breast cancer patients is rising and has become a major clinical challenge. Only few data are published concerning risk factors for the development of CNS metastases as a first site of metastatic disease in breast cancer patients. Moreover, the incidence of CNS metastases after modern neoadjuvant treatment is not clear.
Methods: We analyzed clinical factors associated with the occurrence of CNS metastases as the first site of metastatic disease in breast cancer patients after neoadjuvant treatment in the trials GeparQuinto and GeparSixto (n = 3160) where patients received targeted treatment in addition to taxane and anthracycline-based chemotherapy.
Results: After a median follow-up of 61 months, 108 (3%) of a total of 3160 patients developed CNS metastases as the first site of recurrence and 411 (13%) patients had metastatic disease outside the CNS. Thirty-six patients (1%) developed both CNS metastases and other distant metastases as the first site of metastatic disease. Regarding subtypes of the primary tumor, 1% of luminal A-like (11/954), 2% of luminal B-like (7/381), 4% of HER2-positive (34/809), and 6% of triple-negative patients (56/1008) developed CNS metastases as the first site of metastatic disease.
In multivariate analysis, risk factors for the development of CNS metastases were larger tumor size (cT3–4; HR 1.63, 95% CI 1.08–2.46, p = 0.021), node-positive disease (HR 2.57, 95% CI 1.64–4.04, p < 0.001), no pCR after neoadjuvant chemotherapy (HR 2.29, 95% CI 1.32–3.97, p = 0.003), and HER2-positive (HR 3.80, 95% CI 1.89–7.64, p < 0.001) or triple-negative subtype (HR 6.38, 95% CI 3.28–12.44, p < 0.001).
Conclusions: Especially patients with HER2-positive and triple-negative tumors are at risk of developing CNS metastases despite effective systemic treatment. A better understanding of the underlying mechanisms is required in order to develop potential preventive strategies.
Soluble Triggering Receptor Expressed on Myeloid Cells 1 (sTREM-1) can be found in the sera of patients with infectious, autoimmune and malignant diseases. The primary objective of this study was to investigate the prognostic significance of sTREM-1 in lung cancer patients. We analyzed the sera of 164 patients with lung cancer of all histologies and all stages at the time of diagnosis. We employed an ELISA using the anti-TREM-1 clone 6B1.1G12 mAb and recombinant human TREM-1. Patient data was collected retrospectively by chart review. In ROC-analysis, a sTREM-1 serum level of 163.1 pg/ml showed the highest Youden-Index. At this cut-off value sTREM-1 was a marker of short survival in patients with NSCLC (median survival 8.5 vs. 13.3 months, p = 0.04). A Cox regression model showed stage (p < 0.001) and sTREM-1 (p = 0.011) to indicate short survival. There were no differences in sTREM-1 serum values among patients with or without infection, pleural effusion or COPD. sTREM-1 was not associated with metastasis at the time of diagnosis and was not a predictor of subsequent metastasis. In SCLC patients sTREM-1 levels were lower than in NSCLC patients (p = 0.001) and did not predict survival. sTREM-1 did not correlate with CRP or the number of neutrophils. In non-small cell lung cancer patients, sTREM-1 in serum has prognostic significance.
Korneale Ablationsverfahren zur Korrektur von Myopie, Hyperopie und Astigmatismus bis mittleren Grades haben sich in der refraktiven Chirurgie als sicheres und wirksames Verfahren etabliert. Neben diesen einfachen Abbildungsfehlern gibt es noch Abbildungsfehler höherer Ordnung, welche die Abbildungsqualität des optischen Systems Auge reduzieren. Bisher wurden diese Abbildungsfehler durch konventionelle Excimerlaserbehandlungen induziert. Mit der Entwicklung von Aberrometern soll es nun gelingen, die Induktion von höheren Abbildungsfehlern bzw. vorhandene höhere Abbildungsfehler zusätzlich zur Korrektur des Refraktionsfehlers zu egalisieren. Man erwartet dadurch eine deutliche Verbesserung der Abbildungsqualität des Auges. In dieser Arbeit wurden 97 Augen von 49 Probanden mit einer wellenfrontgeführten LASIK behandelt. Die Untersuchung und Auswertung erfolgte hinsichtlich der Standardparameter der refraktiven Chirurgie (Sicherheit, Wirksamkeit, Stabilität, Vorhersagbarkeit, Komplikationen) sowie hinsichtlich der Veränderungen der Abbildungsfehler höherer Ordnung. Die Untersuchungen fanden präoperativ, nach einem Tag, einer Woche, einem Monat, nach drei Monaten und einem Jahr statt. Die Messung des Wellenfrontfehlers erfolgte präoperativ und nach einem Jahr in Miosis und Mydriasis. Ein Jahr nach der wellenfrontgesteuerten LASIK gewannen 40% der Augen eine Zeile und 5% zwei Zeilen an unkorrigiertem Sehvermögen. Bei 49% blieb das unkorrigierte Sehvermögen unverändert, 6% verloren eine Zeile. Nach einem Jahr wiesen 83% der Augen ein UKSM von 1,0 oder mehr auf. In einem Fall fand sich ein unkorrigiertes Sehvermögen von 1,6. Die Vorhersagbarkeit bis zu zwölf Monaten postoperativ erwies sich als sehr hoch. Alle Augen lagen zu diesem Zeitpunkt innerhalb einer Spannweite von ± 2 dpt, 86% innerhalb von ± 1 dpt und 63% im Bereich von ± 0,5 dpt der angestrebten refraktiven Korrektur. Im Nachbeobachtungszeitraum kam es zu keinen größeren Veränderungen der Refraktion. Am letzten Kontrolltermin ergab sich ein mittleres sphärisches Äquivalent von -0,25 dpt. Intraoperativ kam es zu keinen Komplikationen. In der Frühphase des Nachbeobachtungszeitraums kam es bei 21 Augen zu einer diffusen lamellären Keratitis (DLK) leichten bis mittleren Grades, die aber unter lokaler Kortisonbehandlung folgenlos ausheilte. Im Mittel betrugen die Aberrationen höherer Ordnung („higher order aberrations“- HOA) vor der Behandlung 0,093±0,032 μm für die 3,5 mm Pupille und 0,395±0,134 μm für die 6 mm Pupille. Durch die wellenfrontgeführte LASIK-Behandlung erhöhten sich die Werte auf 0,108±0,05 μm bzw. 0,571±0,244 μm. Somit konnten für eine 3,5 mm Pupille in 45,6% der Augen und in 20,6% der Augen für eine 6 mm Pupille die Abbildungsfehler höherer Ordnung konstant gehalten bzw. verringert werden. Die größte Zunahme der Aberrationen höherer Ordnung zeigte sich für die sphärischen Aberrationen (Z0 4) beim Vergleich der gemessenen Pupillen von 3,5 und 6 mm. Die Vorhersagbarkeit der Abbildungsfehler höherer Ordnung war sehr gering. Alle postoperativen Veränderungen der Abbildungsfehler höherer Ordnung zeigten eine negative Korrelation mit dem präoperativen Wert. Die Ergebnisse der wellenfrontgeführten LASIK erweisen sich hinsichtlich Sicherheit, Wirksamkeit, Stabilität und Vorhersagbarkeit als sehr gut. Dagegen ist die Korrektur der Abbildungsfehler höherer Ordnung noch unzureichend. In keinem Fall konnte der Wellenfrontfehler vollständig korrigiert werden, bei einigen Patienten konnte er zumindest reduziert werden. Technische Weiterentwicklungen und weitere Erfahrungen in der Anwendung dieses Verfahrens sind notwendig, um die Korrektur der höheren Abbildungsfehler zu verbessern.
