Hintergrund: Die klinische Wirksamkeit einer pharmakochirurgischen Kombinationstherapie zur Behandlung des diabetischen Makulaödems (DMÖ) bestehend aus einer posterioren Kernvitrektomie mit Entfernung von 1,5 ml liquefiziertem Glaskörper und zeitgleicher isovolumetrischer Injektion von 1,25 mg Bevacizumab und 8 mg Triamcinolon wurde in der vorliegenden Arbeit analysiert. Methodik: Daten von 73 Augen (60 Patienten; 65,4 ± 10,4 Jahre) mit einem DMÖ nach pharmakochirurgischer Kombinationstherapie wurden analysiert. Untersucht wurde neben dem Visus und der zentralen Makuladicke die Notwendigkeit von Re-Interventionen und dem Einsatz der konventionellen Verfahren (Laser, klassische 3port ppV). Wir unterschieden zwischen Gruppe I (n=38): nicht-proliferative DR (NPDR) ohne ischämische Makulopathie (I.M.); Gruppe II (n=17): NPDR mit I.M. und Gruppe III (n=18): proliferative DR mit oder ohne I.M. Die Kontrollintervalle betrugen im Median 9 Wochen (ca. 2 Monate, T1), 25 Wochen (ca. 6 Monate, T2) und 44 Wochen (ca.10 Monate, T3) nach Intervention. Ergebnisse: Die präoperativen Sehschärfen betrugen 0,52 ± 0,21 logMAR in Gruppe I, 0,99 ± 0,5 logMAR in Gruppe II, 0,77 ± 0,42 logMAR in Gruppe III. In Gruppe I verbesserte sich der Visus nach 2 Monaten zunächst auf 0,45 ± 0,21 logMAR (T1; p=0,7), nach 6 Monaten (T2) hochsignifikant auf 0,36 ± 0,16 logMAR (p<0,01) und blieb zum Zeitpunkt T3 stabil (0,36 ± 0,1 logMAR; p=1,0). Die Gruppe II zeigte zum Zeitpunkt T1 eine signifikante Visusbesserung auf 0,64 ± 0,18 logMAR (p<0,05) und war mit 0,77 ± 0,43 logMAR (T2; p>0,05) und 0,82 ± 0,4 logMAR (T3; p>0,05) leicht rückläufig. In der Gruppe III wurde zum Zeitpunkt T1 ein nicht-signifikante Visusverbesserung auf 0,53 ± 0,24 logMAR (p=0,08) erreicht, während nach 6 Monaten einen Visus von 0,62 ± 0,29 logMAR (p=0,6) und nach 10 Monaten mit 0,72 ± 0,34 logMAR (p=0,9) nahezu den Ausgangsvisus erreicht wurde. Die präoperativen Makuladicken betrugen 386,6 μm in Gruppe I, und 418,65 μm in Gruppe II und 385,17 μm in Gruppe III. Das Makulaödem konnte in allen 3 Gruppen bereits nach 2 Monaten hochsignifikant reduziert: -110μm (-28%; p<0,01) und im erreichten Niveau stabilisiert werden. 27,4 % der Augen (20/73) benötigten nach durchschnittlich 6,3 Monate eine Re-Intervention und 32,9 % benötigten im Verlauf eine Lasertherapie. Schlussfolgerung: Durch die pharmakochirurgische Kombinationstherapie konnte kurz- und mittelfristig eine Reduktion des Makulaödems mit Visusverbesserung in der Mehrzahl der Augen erreicht werden. Die Notwendigkeit von Re-Interventionen konnte zudem signifikant reduziert werden. Durch einen möglichen synergistischen Effekt kann diese Behandlungsmethode bestehende Therapieoptionen sinnvoll ergänzen und erweitern.
RNA-Aptamere sind kurze einzelsträngige Oligonukleotide, die ein Zielmolekül spezifisch erkennen und über ihre 3D-Struktur binden. Die Identifizierung von Aptameren erfolgt mittels in vitro Selektion nach dem Kriterium einer hohen Bindungsaffinität und/oder -spezifität. Das Tetracyclin-bindende Aptamer gehört zu den Aptameren mit der höchsten bekannten Liganden-Affinität. Darüber hinaus gehört es zu den wenigen RNA-Aptameren, die in vivo als artifizieller Riboswitch zur Modulation der Genexpression eingesetzt werden können. Im Rahmen der vorliegenden Arbeit wurde das Tetracyclin-bindende Aptamer mittels NMR-spektroskopischer und biophysikalischer Methoden im ligandfreien sowie im ligandgebundenen Zustand untersucht, um neben der bereits bekannten Kristallstruktur des RNA-Tetracyclin-Komplexes Aufschlüsse über den dynamischen Ligandenbindungsprozess und die damit verbundene genregulatorische Aktivität des Aptamers zu erhalten. Hierfür wurde der Einfluss von Mg2+-Ionen auf die globale und lokale Strukturausbildung des ligandfreien Aptamers analysiert. Durch die Bindung von Mg2+-Ionen an die RNA wird eine kompakte RNA-Struktur stabilisiert, die neben zahlreichen komplexen Tertiärinteraktionen eine starre Bindungstasche ausbildet, in der der Ligand nach einem „Schlüssel-Schloss-Prinzip“ bindet. Die Ligandbindung zieht nur noch kleinere strukturelle Änderungen nach sich. Mittels der Stopped-Flow-Technik wurden die kinetischen Aspekte der Ligandbindung in Abhängigkeit der Mg2+-Konzentration untersucht. Diese Methode ermöglichte die Analyse der Zusammenhänge zwischen RNA-Faltung und anschließender Komplexbildung in Echtzeit. Die Analyse der Stopped-Flow-Messungen ergab, dass die Geschwindigkeit des Ligandbindungsprozesses wesentlich von der Mg2+-induzierten Strukturausbildung abhängig ist. Die Mg2+-vermittelte globale Organisation der RNA-Struktur ist somit der geschwindigkeitsbestimmende Schritt des Ligandbindungsprozesses. Die RNA-Mg2+-Interaktionen bestimmen also nicht nur die 3D-Struktur des Tetracyclin-bindenden Aptamers, sondern auch die Kinetik des Ligandbindungsprozesses. Der detaillierte Vergleich des Tetracyclin-Aptamers in seiner ligandfreien und ligandgebundenen Form ergab, dass trotz der stark ausgeprägten strukturellen Ähnlichkeit, lediglich die ligandgebundene Form in einer thermisch stabilen Konformation vorliegt. Die signifikante Erhöhung der Thermostabilität durch die Ligandbindung ist die essentielle Voraussetzung für die genregulatorische Funktion des Aptamers. Basierend auf diesen Ergebnissen ist also nicht die Struktur, sondern die strukturelle Stabilität ausschlaggebend für die regulatorische Aktivität des Tetracyclin-bindenden Aptamers.
Ein weiterer Teil dieser Arbeit beschäftigt sich mit der Charakterisierung des Bindungsmodus von GTP an die GTP-bindenden Aptamere 9-4, 10-10, Klasse II und Klasse V. Durch den direkten NMR-spektroskopischen Nachweis von Wasserstoffbrückenbindungen konnte eine intermolekulare G:C-Watson-Crick-Basenpaarung zwischen GTP und den GTP-bindenden Aptameren 9-4, 10-10 und Klasse II gezeigt werden. Basierend auf diesen Ergebnissen konnte durch eine C zu U Mutation die Bindungsspezifität des Aptamers Klasse II von GTP zu 2-Amino-ATP verändert werden.
Weiterhin konnte im Rahmen dieser Arbeit ein intermolekularer G-Quadruplex als Ligandbindungsmodus zwischen GTP und dem GTP-bindenden Aptamer Klasse V beschrieben werden. Hierbei bildet GTP mit sieben Guanin-Basen der RNA eine intermolekulare G-Quadruplexstruktur, bestehend aus zwei übereinanderliegenden Guanin-Tetraden, aus. Durch den Einsatz von 15N-markiertem NH4+ konnte eine spezifische Kaliumbindungsstelle im Zentrum der Quadruplexstruktur lokalisiert werden, die zur Stabilisierung des RNA-Ligand-Komplexes dient. Die beobachteten NOE-Kreuzsignale zwischen den Protonen des gebundenen NH4+ und den Protonen der RNA bestätigten dabei die Ausbildung eines intermolekularen G-Quadruplexes. Zusätzlich ergab die Analyse der NMR-Spektren, dass die G-Quadruplexstruktur erst im Zuge der Ligandbindung ausgebildet wird. Die Bildung eines G-Quadruplexes, in der der Ligand einen integralen Bestandteil der Quadruplexstruktur darstellt, ist ein bislang unbeschriebener Bindungsmechanismus.
Untersuchung von Rezeptor-Ligand-Komplexen mittels organischer Synthese und NMR-Spektroskopie

Viele biologische Prozesse basieren auf der spezifischen Bindung eines Liganden an einen Rezeptor. Die Wechselwirkung zwischen dem Rezeptor und seinem Ligand kann im Wesentlichen durch zwei verschiedene Modelle beschrieben werden: zum einen das vom E. Fischer eingeführte Schlüssel-Schloss-Prinzip und zum anderen das von Koshland beschriebene "induced-fit-model". Bei dem Schlüssel-Schloss-Prinzip liegt der Ligand in der Bindetasche des Rezeptors wie ein Schlüssel im Schloss. Ganz anders hierzu setzt die induzierte Anpassung ("induced-fit-model") eine konformationelle Änderung des Proteins durch den Liganden für die Bindung voraus. Ändern sich jedoch die Konformationen von Substrat und Rezeptor in einer gegenseitigen Beeinflussung, dann spricht man von "double-induced-fitmodel". Die Untersuchung dieser Erkennung auf molekularer Ebene ist von großer Wichtigkeit, denn sie dient zum besseren Verständnis und damit auch zur gezielten Beeinflussung solcher Prozesse. Wie wird der Ligand von einem Rezeptor selektiv erkannt und gebunden? Für die Erkennung und Bindung spielen spezifische nichtkovalente Wechselwirkungen eine wichtige Rolle. Zum Repertoire der nichtkovalenten Wechselwirkungen gehören die elektrostatische Wechselwirkungen, die Wasserstoffbrückenbindung und der hydrophobe Effekt.
In der vorliegenden Arbeit werden anhand von drei ausgewählten Beispielen solche Wechselwirkungen zwischen verschiedenen Liganden mit ihrem Rezeptor untersucht. In den ersten beiden Kapiteln werden Proteine und im letzten Kapitel RNA als Rezeptor untersucht. Die einzelnen Kapitel beginnen jeweils mit einer kurzen Einführung der Rezeptoren und der dazugehörenden Liganden, schließlich wird dann die Rezeptor-Ligand-Wechselwirkung beschrieben. Als Rezeptor wurden in der vorliegenden Arbeit Proteine (Kinasen und Membranproteine) und strukturierten Elemente der RNA (Aptamerdomäne der purinbindenden Riboswitche und der SELEX-RNA) gewählt. Membranproteine der Atmungskette, Kinasen und Riboswitches stellen zusätzlich attraktive Rezeptoren für das Wirkstoffdesign dar. Die damit interferierenden Liganden umfassen Substrate, Cofaktoren, Metabolite und Inhibitoren. Die Untersuchung der Wechselwirkung erfolgte mittels NMR-Spektroskopie und organischer Synthese.
An overexpression of the E3 ubiquitin ligase TRIM25 is implicated in several human cancers and frequently correlates with a poor prognosis and occurrence of therapy resistance in patients. Previous studies of our group have identified the mRNA encoding the pro-apoptotic caspase-2 as a direct target of the ubiquitous RNA binding protein human antigen R (HuR). The constitutive HuR binding observed in colon carcinoma cells negatively interferes with the translation of caspase-2 mainly through binding to the 5' untranslated region (UTR) of caspase-2 and thereby confers an increased survival of tumor cells. The main objective of this thesis was to unravel novel regulatory proteins critically involved in the control of caspase-2 translation and their impact on therapeutic drug resistance of human colon carcinoma cells. By employing RNA affinity chromatography in combination with mass-spectrometry, among several putative caspase-2 mRNA binding proteins, we have identified the tripartite motif-containing protein 25 (TRIM25) as novel caspase-2 translation regulatory protein in colon carcinoma cells. The constitutive TRIM25 binding to caspase-2 mRNA in two different human colorectal carcinoma cell lines was validated by ribonucleoprotein (RNP)-immunoprecipitation (RIP)-RT-PCR assay and by means of biotin-labeled RNA-pull-down assay. Since caspase-2 is a caspase which is particularly involved in the DNA-damage-induced apoptosis, I tested the functional relevance of negative caspase-2 regulation by TRIM25 for chemotherapeutic drug-induced cell death of different adenocarcinoma cells by RNA interference (RNAi)- mediated loss-of-function and gain-of-function approaches. In the first part of the thesis, I could demonstrate that transient silencing of TRIM25 caused a significant increase in caspase-2 protein levels without affecting the amount of corresponding mRNAs. Mechanistically, the TRIM25 silencing-triggered increase in caspase-2 was totally impaired by cycloheximide, indicating that the stimulatory effects on caspase-2 levels depend on protein synthesis. This finding was corroborated by RNP/polysomal fractionation, which revealed that the transient knockdown of TRIM25 caused a significant redistribution of caspase-2 transcripts from the fraction of RNP particles to that from translationally active polyribosomes.
The second part of my thesis aimed at the elucidation of the functional consequences of the negative caspase-2 regulation by TRIM25 for enhanced tumor cell survival. Thereby, I found that the siRNA-mediated knockdown of TRIM25 caused a significant increase in the chemotherapeutic drug-induced cleavage of caspase-3 and to elevations in cytoplasmic cytochrome c levels implicating that TRIM25 depletion did mainly affect the intrinsic apoptotic pathway. Concordantly, the ectopic expression of TRIM25 caused a reduction in caspase-2 protein levels, concomitant with an attenuated sensitivity of tumor cells to doxorubicin.
To test the functional impact of caspase-2 in the TRIM25 depletion-dependent sensitization to drug-induced apoptosis, I employed a siRNA-mediated knockdown of caspase-2. Interestingly, the strong induction of caspase-3 and -7 cleavage after doxorubicin treatment was fully impaired after the additional knockdown of caspase-2, indicating the sensitizing effects by TRIM25 knockdown depend on caspase-2.
Data from this thesis identified the TRIM25 as a novel RNA-binding protein of caspase-2 mRNA, which negatively interferes with the translation of caspase-2 and which functionally contributes to chemotherapeutic drug resistance of colon carcinoma cells. Interfering with the negative TRIM25-caspase-2 axis may represent a promising therapeutic avenue for sensitizing colorectal cancers to conventional anti-tumor therapies.
Primäre Tumore werden nach ihrem Entstehungsort benannt. Selbst Metastasen zeigen eine gewisse Ähnlichkeit mit ihrem ursprünglichen Gewebe. Somit gehören alle Geschwülste, die ursprünglich der Harnblase entstammen, zu den Harnblasenkarzinomen.
Das Harnblasenkarzinom geht meist (90-95%) von der Schleimhaut der ableitenden Harnwege aus und wird als Urothel bezeichnet. Dementsprechend haben die meisten Patienten mit der Diagnose Blasenkarzinom ein Urothel-Blasenkarzinom. Die restlichen Blasenkarzinome entfallen auf Adenokarzinome, Plattenepithelkarzinome, kleinzellige Karzinome, Sarkome, Paragangliome, Melanome oder Lymphome (Humphrey A. et al. 2016, Moch H. et al 2016).
Die Urothel-Blasenkarzinome können sowohl flach, als auch warzenförmig wachsen. Je nach Diagnostik „oberflächlich“ oder „muskelinvasiv“ lassen sich die Urothel-Blasenkarzinome in zwei Hauptgruppen unterteilen;
Etwa 70% der Erkrankten haben dabei ein oberflächliches Urothel-Blasenkarzinom, das auf die Blasenschleimhaut begrenzt ist und durch eine Basistherapie, sog. Transurethrale Resektion (TUR-B) behandelt wird. Dabei werden die in der Schleimhaut gewachsenen Tumore getrennt reseziert. Therapieergänzend und/oder prophylaktisch wird die Blase danach mit einem Chemotherapeutikum gespült (intravesikale Instillation). Diese Zytostatika-Behandlung soll das Wiederauftreten eines Rezidivs verhindern bzw. eventuell verlangsamen (Iida K. et al. 2016, Celik O. et al. 2016).
The problem of unconstrained or constrained optimization occurs in many branches of mathematics and various fields of application. It is, however, an NP-hard problem in general. In this thesis, we examine an approximation approach based on the class of SAGE exponentials, which are nonnegative exponential sums. We examine this SAGE-cone, its geometry, and generalizations. The thesis consists of three main parts:
1. In the first part, we focus purely on the cone of sums of globally nonnegative exponential sums with at most one negative term, the SAGE-cone. We ex- amine the duality theory, extreme rays of the cone, and provide two efficient optimization approaches over the SAGE-cone and its dual.
2. In the second part, we introduce and study the so-called S-cone, which pro- vides a uniform framework for SAGE exponentials and SONC polynomials. In particular, we focus on second-order representations of the S-cone and its dual using extremality results from the first part.
3. In the third and last part of this thesis, we turn towards examining the con- ditional SAGE-cone. We develop a notion of sublinear circuits leading to new duality results and a partial characterization of extremality. In the case of poly- hedral constraint sets, this examination is simplified and allows us to classify sublinear circuits and extremality for some cases completely. For constraint sets with certain conditions such as sets with symmetries, conic, or polyhedral sets, various optimization and representation results from the unconstrained setting can be applied to the constrained case.
In this thesis the Quadrupole Electrical Resistivity Tomography (QERT) method is presented as a new measurement concept for profile-based geoelectric field measurements. The concept is based on a tensorial formulation of the apparent resistivity in order to make three-dimensional statements about the underground conductivity structure. For a simple application of the method a number of similarities to the classical dipole-dipole method were made, such as the presentation of the measurement data in a pseudo-section. The added value of the method compared to the classical profile-based methods is especially the differentiation of lateral structures. Anomalies, which are located laterally to the profile, can be detected with respect to their position (left-right) as well as their conductivity contrast. For the practical implementation of the concept a measuring device was developed and constructed, the CR Device. The device uses 64 channels for simultaneous signal recording of voltage and current time series with up to 1 kHz sampling rate. The current injection is freely programmable and allows any survey design. The measurement of the voltages is performed against a common reference (CR) electrode and thus allows the reconstruction of any dipole voltage by difference formation. A complementary, Matlab-based software package completes the measuring system. An evaluation module allows the raw data of the CR device to be read in, processed and displayed in a suitable form. An inversion module allows the inversion of measurement data into a three-dimensional subsurface model. With a modeling module, measurements over any subsurface situation can be simulated and subsequently analysed. A field measurement on a volcanic maar in the Eifel region, Germany, demonstrates the benefits of the method. A QERT profile was set-up tangentially to a conductive anomaly in the centre of the maar. The measurement data were successfully inverted into a geologically coherent 3D resistivity model.
The postthrombotic syndrome (PTS) is beside the venous thromboembolism (VTE) recurrence and chronic thromboembolic pulmonary hypertension (CTEPH) a long-term adverse outcome and chronic complication of deep vein thrombosis (DVT) in the lower extremities and can occur in up to 20–50% of patients within 2 years after DVT. The prevalence of PTS in the adult population is expected to increase due to the growing incidence of VTE in the elderly. Although not life threatening it can impose significant morbidity and can be associated with a negative impact on quality of life associated with disease severity. From an economic point of view, PTS is an important predictor of increased health care costs after VTE.
Factors potentially related to the development of the PTS are older age, obesity, a history of previous ipsilateral DVT, iliofemoral location of the current thrombosis, failure to promptly recover from the acute symptoms and insufficient quality of oral anticoagulant therapy. Furthermore, it is known that the severity of PTS correlates with the location of the DVT, the more proximal the more severe.
PTS induces a range of symptoms and clinical signs, which can be assessed in different scales. The Villalta scale is one of the most suitable scales for defining the presence and severity of subjective symptoms and physical signs of PTS.
In the last century, various therapeutic strategies have been developed to prevent mortality due to VTE or long-term morbidity due to PTS.
Conservative treatment today consists of anticoagulation - usually using direct oral anticoagulants - and compression therapy. One of the first invasive treatments with the aim of thrombus removal was surgical venous thrombectomy by Läwen in 1938. Mahorner and Fontaine improved the technique in the 1950s combining it with a course of anticoagulant treatment to prevent rethrombosis and PTS.
Mechanical thrombectomy by the use of Fogarty balloons, which started in 1963, or the creation of a transient arteriovenous fistula, performed since 1974, are now no longer recommended due to the high invasiveness, risk of fatal intraoperative embolism and a high rethrombosis rate.
In current practice, early thrombus removal mainly relies on the use of catheter-directed pharmacologic thrombolytic therapy. Another approach currently is the endovenous, device-driven thrombectomy and stenting in case of venous obstruction. There is an ongoing broad discussion as to whether these invasive therapies should be offered to patients with iliofemoral thrombosis (IFT), which remains controversial.
IFT, the major target for endovenous thrombectomy respectively pharmacologic thrombolytic therapy, is not enough represented in current literature because the used definition of proximal DVT does not necessarily include the iliac veins. In consequence, it may not be representative enough concerning questions like prevalence and severity of PTS or the effects on quality of life.
The present registry – the Iliaca-PTS registry – addresses exactly these patients and tries to answer these questions. The data of 85 patients who had suffered an IFT in the past were evaluated in the prospective registry documenting the severity of PTS, the occurrence of iliac vein compression syndrome in left-sided IFT and quality of life. A significant predictor for the development of severe PTS or venous claudication in our patient population is a high BMI.
The results of this registry show that IFT is frequently observed and only ten percent develop a moderate or severe PTS respectively venous claudication. In conclusion, the conservative treatment strategy with optimal effective anticoagulant therapy can lead to a low incidence of PTS and a high quality of life.
Mit der vorliegenden Arbeit wird für den südöstlichen Taunus und sein Vorland erstmals eine umfassende monographische Bearbeitung von Flora und Vegetation des Grünlands auf Basis umfangreicher Geländeerhebungen und Literaturrecherchen vorgelegt. Die wesentlichen Ziele der Untersuchung sind: • Darstellung der aktuellen und historischen Vorkommen und räumlichen Verbreitung von Pflanzenarten, Pflanzengesellschaften und Nutzungsintensitäten des Grünlands. • Darstellung der historischen Entwicklung des Grünlands und der sozioökonomischen Situation der Landwirtschaft. • Gefärdungseinstufung der Pflanzenarten und -gesellschaften (Rote Liste). • Kritische Bewertung des derzeitigen Stands der floristisch-vegetationskundlichen Landesforschung. • Bereitstellung von fachlichen Grundlagen für den praktischen Naturschutz, für Naturschutzbehörden, Planungsbüros, regionale Naturschutzforschung und die interessierte Öffentlichkeit. Das 1105 km2 große Untersuchungsgebiet liegt in nordwestlichen Rhein-Main-Gebiet und erstreckt sich von Wiesbaden im Südwesten und Bad Nauheim im Nordosten bzw. Schmitten im Nordwesten und Frankfurt im Südosten. Es umfasst mit dem Hebungsgebiet des Taunus (größte Höhe 878,5 m ü. NN) und dem Senkungsgebiet des Rhein-Main-Tieflands (tiefster Punkt 84 m ü. NN) zwei sehr unterschiedliche geowissenschaftliche Landschaftstypen, die im einzelnen 33 verschiedene naturräumliche Teileinheiten umfassen...
