Background: Since sorafenib has shown activity in different tumour types and gemcitabine regimens improved the outcome for biliary tract cancer (BTC) patients, we evaluated first-line gemcitabine plus sorafenib in a double-blind phase II study.
Patients and methods: 102 unresectable or metastatic BTC patients with histologically proven adenocarcinoma of gallbladder or intrahepatic bile ducts, Eastern Cooperative Oncology Group (ECOG) 0–2 were randomised to gemcitabine (1000 mg/m2 once weekly, first 7-weeks + 1-week rest followed by once 3-weeks + 1-week rest) plus sorafenib (400 mg twice daily) or placebo. Treatment continued until progression or unacceptable toxicity. Tumour samples were prospectively stained for sorafenib targets and potential biomarkers. Serum samples (first two cycles) were measured for vascular endothelial growth factors (VEGFs), vascular endothelial growth factor receptor 2 (VEGFR-2) and stromal cell-derived factor 1 (SDF1)α by enzyme-linked immunosorbent assay (ELISA).
Results: Gemcitabine plus sorafenib was generally well tolerated. Four and three patients achieved partial responses in the sorafenib and placebo groups, respectively. There was no difference in the primary end-point, median progression-free survival (PFS) for gemcitabine plus sorafenib versus gemcitabine plus placebo (3.0 versus 4.9 months, P = 0.859), and no difference for median overall survival (OS) (8.4 versus 11.2 months, P = 0.775). Patients with liver metastasis after resection of primary BTC survived longer with sorafenib (P = 0.019) compared to placebo. Patients who developed hand-foot syndrome (HFS) showed longer PFS and OS than patients without HFS. Two sorafenib targets, VEGFR-2 and c-kit, were not expressed in BTC samples. VEGFR-3 and Hif1α were associated with lymph node metastases and T stage. Absence of PDGFRβ expression correlated with longer PFS.
Conclusion: The addition of sorafenib to gemcitabine did not demonstrate improved efficacy in advanced BTC patients. Biomarker subgroup analysis suggested that some patients might benefit from combined treatment.
Wir untersuchten über einen Zeitraum von 5 Jahren 40 Patienten, bei denen die Diagnose einer Achalasie nach etablierten Kriterien gesichert worden war hinsichtlich der Effektivität und der Nebenwirkungen einer pneumatischen Dilatation mit einem neuartigen "Low - Compliance" Ballonsystem. Alle Patienten wurden vor, 4 - 6 Wochen und abschließend im Mittel 28 ± 15 Monate nach der pneumatischen Dilatation untersucht. Die pneumatische Dilatation erfolgte entweder unter Verwendung eines 30 oder 35 mm Ballonsystemes, abhängig von vorher festgelegten Kriterien. Zusätzlich konnte bei 12 dieser Patienten, vor - und im Mittel 26 ± 15 Monate nach der pneumatischen Dilatation, eine ösophageale pH - Messung durchgeführt werden. Insgesamt wurden 52 Dilatationen mit einer durchschnittlichen Dilatationszeit von 4 Minuten durchgeführt. Ein initialer Erfolg nach einmaliger pneumatischer Dilatation konnte bei 35 Patienten (87,5%) erreicht werden. Die übrigen 5 Patienten sowie weitere 7 Patienten, bei denen eine Dysphagie erneut auftrat, wurden ein zweites Mal dilatiert. Die beiden Patienten, bei denen auch nach dieser zweiten Dilatation noch deutliche Symptome bestanden, wurden einer operativen Kardiomyotomie zugeführt (Operationsfrequenz = 5%). Die Effektivität der Dilatation wurde durch eine signifikante Verringerung der klinischen Symptomatik dokumentiert. So verringerte sich der Symptomen - Score (1-14 Punkte) von 9,9 ± 2,1 Punkte vor pneumatischer Dilatation, auf 4,9 ± 2,4 Punkte 4-6 Wochen nach pneumatischer Dilatation und auf 4,9 ± 2,4 Punkte bei der Abschlußuntersuchung (p< 0,01). Der Ruhedruck des unteren Ösophagussphinkters konnte durch die Dilatation von 28,3 ± 0,8 mmHg auf 16,4 ± 6,4 mmHg nach Dilatation und weiter auf 14,7 ± 5,5 mmHg bei der Abschlußuntersuchung, gesenkt werden (p< 0,01). Weder der radiologisch bestimmte Durchmesser des tubulären Ösophagus, noch die manometrisch fassbaren Kontraktionsamplituden im tubulären Ösophagus wurden durch die pneumatische Dilatation signifikant beeinflußt. Betrachtet man die Nebenwirkungen, so zeigte die Langzeit pH - Metrie eine deutliche Erhöhung der Anzahl, sowie der Dauer der gastroösophagealen Refluxepisoden, mit einem pH < 4 nach der pneumatischen Dilatation. Konträr zu diesen Ergebnissen klagte allerdings nur ein Patient über Sodbrennen. Bei einem weiteren Patienten konnte, obwohl dieser keine Beschwerden angab, endoskopisch eine Ösophagitis Grad I gesichert werden. Es wurde eine Perforation beobachtet, eine operative Versorgung des Patienten war jedoch nicht erforderlich. Diese Ergebnisse zeigen, daß die pneumatische Dilatation mit dem "Low - Compliance" System eine adäquate Behandlungsmethode der Achalasie darstellt, die sich jedoch hinsichtlich ihrer Kurz - und Langzeiteffektivität nicht wesentlich von den bisher verwendeten Systemen unterscheidet.
The transcription factor Meis1 drives myeloid leukemogenesis in the context of Hox gene overexpression but is currently considered undruggable. We therefore investigated whether myeloid progenitor cells transformed by Hoxa9 and Meis1 become addicted to targetable signaling pathways. A comprehensive (phospho)proteomic analysis revealed that Meis1 increased Syk protein expression and activity. Syk upregulation occurs through a Meis1-dependent feedback loop. By dissecting this loop, we show that Syk is a direct target of miR-146a, whose expression is indirectly regulated by Meis1 through the transcription factor PU.1. In the context of Hoxa9 overexpression, Syk signaling induces Meis1, recapitulating several leukemogenic features of Hoxa9/Meis1-driven leukemia. Finally, Syk inhibition disrupts the identified regulatory loop, prolonging survival of mice with Hoxa9/Meis1-driven leukemia.
