Pro-resolving lipid mediator Resolvin D1 serves as a marker of lung disease in cystic fibrosis

Background: Cystic fibrosis (CF) is an autosomal recessive genetic disorder that affects multiple organs, including the lungs, pancreas, liver and intestine. Mutations in the cystic fibrosis transmembrane conductance regulator (CFTR) locus lead to defective proteins and reduced Cl- secretion and Na+ hyperabsorption in the affected organs. In addition, patients suffering from CF display chronic inflammation that contributes to the pathogenesis of CF. Recent work suggests that CF patients have a reduced capacity to biosynthesize specialized pro-resolving lipid mediators (SPMs), which contributes to the development and duration of the unwanted inflammation. Alterations in the metabolism of arachidonic acid (AA) and docosahexaenoic acid (DHA) to specialized pro-resolving mediators (SPMs), like lipoxins (LXs), maresins (MaRs), protectins (PDs) and resolvins (Rvs), may play a major role on clinical impact of airway inflammation in CF.
Methods: In this study, our aims were to detect and quantitate Resolvin D1 (RvD1) in sputum and plasma from patients with CF and compare levels of RvD1 with biomarkers of inflammation and lung function. We studied 27 CF patients aged 6 to 55 years (median 16 years) in a prospective approach.
Results: DHA can be found in the plasma of our CF patients in the milligram range and is decreased in comparison to a healthy control group. The DHA-derived pro-resolving mediator Resolvin D1 (RvD1) was also present in the plasma (286.4 ± 50 pg/ mL, mean ± SEM) and sputum (30.0 ± 2.6 pg/ mL, mean ± SEM) samples from our patients with CF and showed a positive correlation with sputum inflammatory markers. The plasma concentrations of RvD1 were ten times higher than sputum concentrations. Interestingly, sputum RvD1/ IL-8 levels showed a positive correlation with FEV1 (rs = 0.3962, p< 0.05).
Conclusions: SPMs, like RvD1, are well known to down-regulate inflammatory pathways. Our study shows that the bioactive lipid mediator RvD1, derived from DHA, was present in sputum and plasma of CF patients and may serve as a representative peripheral biomarker of the lung resolution program for CF patients.
Curcumin, the active constituent of Curcuma longa L. (family Zingiberaceae), has gained increasing interest because of its anti-cancer, anti-inflammatory, anti-diabetic, and anti-rheumatic properties associated with good tolerability and safety up to very high doses of 12 g. Nanoscaled micellar formulations on the base of Tween 80 represent a promising strategy to overcome its low oral bioavailability. We therefore aimed to investigate the uptake and transepithelial transport of native curcumin (CUR) vs. a nanoscaled micellar formulation (Sol-CUR) in a Caco-2 cell model. Sol-CUR afforded a higher flux than CUR (39.23 vs. 4.98 μg min−1 cm−2, respectively). This resulted in a higher Papp value of 2.11 × 10−6 cm/s for Sol-CUR compared to a Papp value of 0.56 × 10−6 cm/s for CUR. Accordingly a nearly 9.5 fold higher amount of curcumin was detected on the basolateral side at the end of the transport experiments after 180 min with Sol-CUR compared to CUR. The determined 3.8-fold improvement in the permeability of curcumin is in agreement with an up to 185-fold increase in the AUC of curcumin observed in humans following the oral administration of the nanoscaled micellar formulation compared to native curcumin. The present study demonstrates that the enhanced oral bioavailability of micellar curcumin formulations is likely a result of enhanced absorption into and increased transport through small intestinal epithelial cells.
Background: Lung disease phenotype varies widely even in the F508del (homozygous) genotype. Leukocyte-driven inflammation is important for pulmonary disease pathogenesis in cystic fibrosis (CF). Blood cytokines correlate negatively with pulmonary function in F508del homozygous patients, and gap junction proteins (GJA) might be related to the influx of blood cells into the lung and influence disease course. We aimed to assess the relationship between GJA1/GJA4 genotypes and the clinical disease phenotype. Methods: One-hundred-and-sixteen homozygous F508del patients (mean age 27 years, m/f 66/50) were recruited from the CF centers of Bonn, Frankfurt, and Amsterdam. Sequence analysis was performed for GJA1 and GJA4. The clinical disease course was assessed over 3 years using pulmonary function tests, body mass index, Pseudomonas aeruginosa colonization, diabetes mellitus, survival to end-stage lung disease, blood and sputum inflammatory markers. Results: Sequence analysis revealed one clinically relevant single nucleotide polymorphism. In this GJA4 variant (rs41266431), homozygous G variant carriers (n = 84/116; 72.4%) had poorer pulmonary function (FVC% pred: mean 78/86, p < 0.040) and survival to end-stage lung disease was lower (p < 0.029). The frequency of P. aeruginosa colonization was not influenced by the genotype, but in those chronically colonized, those with the G/G genotype had reduced pulmonary function (FVC% pred: mean 67/80, p < 0.049). Serum interleukin-8 (median: 12.4/6.7 pg/ml, p < 0.052) and sputum leukocytes (2305/437.5 pg/ml, p < 0.025) were higher for the G/G genotype. Conclusions: In carriers of the A allele (27.6%) the GJA4 variant is associated with significantly better protection against end-stage lung disease and superior pulmonary function test results in F508del homozygous patients. This SNP has the potential of a modifier gene for phenotyping severity of CF lung disease, in addition to the CFTR genotype.

Aims: We have provided evidence in former studies that cytokines (IL-8, TNF alpha, LBP, TGFß) measured in blood correlate negatively with lung function in deltaF508 homozygous patients. GAP junction proteins might be of importance for the influx of blood cells into the lung. Our aim was to assess the relationship between connexin genotypes and cytokines (IL-8, TNF-alpha, LBP, TGFß) in induced sputum and serum, and lung disease.
Methods: 36 patients homozygous for deltaF508 (median age 18 y, m/f 16/20, FEV1(%) 77) were examined. Sequence analysis was performed for genes encoding GAP junction protein alpha 1 (GJA1/connexin 43) and gap junction protein alpha 4 (GJA4/connexin 37). Cytokines were assessed in serum and induced sputum (IS) by chemiluminescence (DPC Biermann, Bad Homburg, Germany) as well as leukocyte counts.
Results: DNA analysis was performed in 35 patients. Whereas GJA1 showed only one rare heterozygous synonymous SNP (rs138386744) in one patient, four common SNPs were detected in GJA4. Two were synonymous changes, but the third variant (rs41266431) predicts an amino acid substitution (GTA → valine, ATA → isoleucine) as well as the fourth SNP (rs1764391: CCC→proline, TCC→serine). For rs41266431 patients with homozygosity for the G variant had higher IL-8 levels (median: 13.3/8.0 pg/ml, p=0.07) in serum as well as leukocytes in sputum (median: 2050/421 /µl p=0.041) than those showing heterozygosity (G/A). In individuals > 30 years lung function (FEV1 41.3/84.83 % predicted, p=0.07) was worse.
Conclusion: SNP rs41266431 seems a promising candidate for further investigations, suggesting GJA4 a potential disease modifying gene.
Elche stellen eine Problemart in der Zootierhaltung dar. Sie erreichen im Zoo entgegen dem Trend, dass Tiere in Menschenobhut älter werden als ihre frei lebenden Artgenossen, meist weniger als die Hälfte des biologisch möglichen Alters. Besonders schwierig gestaltet sich die adäquate Fütterung, da die Tiere sehr stark von der Gabe frischer Laubäsung abhängen und die Fütterung mit Ersatzfuttermitteln, wie z.B. Heu, langfristig nicht erfolgsversprechend ist. Zudem sind die Tiere sehr anfällig für Parasiten und eine von Schafen übertragene Viruserkrankung (Bösartiges Katarrhalfieber). Ihr hoher Platzbedarf und das Problem der innerartlichen Aggression machen die Haltung dieser größten Hirschart zusätzlich kompliziert. In dieser Arbeit wird die Methode der Angewandten Chronoethologie in der Zootierhaltung vorgestellt, die sich die von der Inneren Uhr gesteuerten Verhaltensrhythmen einer Tierart zur Beurteilung ihres Wohlbefindens und ihrer Haltungsbedingungen in der künstlichen Zooumwelt zu Nutze macht. Der Besitz einer Inneren Uhr stellt in der hochgradig rhythmisch organisierten Umwelt einen Selektionsvorteil im Sinne einer frühzeitigen Anpassung an wiederkehrende Umweltbedingungen dar. Die Innere Uhr ist so alt wie das Leben selbst, genetisch fixiert, über Zeitgeberreize mit ihrer Umwelt synchronisiert und regelt die Lebensvorgänge von Organismen auf allen organisatorischen Ebenen, auch auf der Ebene des Verhaltens. Ist der normale Verhaltensrhythmus einer Tierart bzw. eines Individuums bekannt, ist es möglich, aus Abweichungen von der Norm auf Störungen des Organismus - auf Unwohlsein - zu schließen. Anhand des Vergleiches von drei Elchhaltungen mit jeweils zwei Tieren, die unter verschiedenen Bedingungen gehalten wurden, konnten mit Hilfe zeitgeraffter Videoaufzeichnung und dem Einsatz eines Speichertelemetriesystems die folgenden Faktoren als einflussreich auf das Verhaltensmuster von Elchen in Menschenobhut identifiziert werden. Die nächtliche Aufstauung von Elchen ist aufgrund des Raum- und Reizmangels in den Boxen ein erheblicher Eingriff in das natürliche, gleichmäßig über Tag und Nacht verteilte Verhaltensmuster der Tiere. Außerdem sind sie in ihrem Verhalten hochgradig vom täglichen Ablauf des Pflegeralltags im Zoo beeinflusst. Feste Fütterungszeiten können einen starken Zeilgeberreiz darstellen- den die Elche in den beiden naturferneren Haltungen stärker zu antizipieren scheinen als den Zeitgeber „Licht". Größe und vor allem Strukturierung des Außengeheges haben einen Einfluss auf dessen zeitliche und räumliche Nutzung. Abschüssige Gehegeteile können die unerwünschte Verhaltensweise Grasen fördern. In unseren mitteleuropäischen Breiten leiden die Elche im Sommer unter Hitzestress, was sich in verringerter Aktivität widerspiegelt. Ein hohes Besucheraufkommen kann die Tiere ebenfalls in ihrem Verhalten beeinflussen. Es hat sich jedoch gezeigt, dass sich der Elch schnell an die Zoogegebenheiten und viele Besucher gewöhnt. Durch den Vergleich der verschiedenen Haltungsbedingungen konnten in der vorliegen Arbeit Vorschläge zur Verbesserung der Haltung von Elchen in Menschenobhut und zu ihrem zeitlichen Management gemacht werden. Der Elch hat sich aufgrund des für Wiederkäuer typischen, sehr geregelten Verhaltensrhythmus aus alternierenden Aktivitäts- und Ruhephasen, der auch unter den verschiedenen Haltungsbedingungen prinzipiell bestehen bleibt, als sehr gut geeignetes Modelltier für die Anwendbarkeit der Methode der Angewandten Chronoethologie in der Zootierhaltung herausgestellt. Abweichungen von der Norm können leicht erkannt werden. Änderungen im Verhaltensmuster m Form erhöhter lokomotorischer Aktivität lassen bei den untersuchten Tieren auf Unwohlsein (Krankheit, Abweichungen von der täglichen Routine, unterbundenes Brunftverhalten) schließen und können im Sinne eines chronoethologischen Paradigmas als eine indikative Verhaltensweise gewertet werden. Das Ziel einer vollständig automatisierten Verhaltenserfassung- und Auswertung mittels Bewegungsmeldern soll im restringierten Raum der Elchbox in einem weiteren Projekt verwirklicht werden, um damit Tierpflegern, Zooveterinären und Verantwortlichen ein gut handhabbares Mittel zur objektiven und langfristigen Beurteilung des Wohlbefindens ihrer Schützlinge zur Verfügung stellen zu können.
