Background: Peritoneal metastasis is a common and dismal evolution of several gastrointestinal (GI) tumors, including gastric, colorectal, hepatobiliary, pancreatic, and other cancers. The therapy of peritoneal metastasis is largely palliative; with the aim of prolonging life and preserving its quality. In the meantime, a significant pharmacological advantage of intraperitoneal chemotherapy was documented in the preclinical model, and numerous clinical studies have delivered promising clinical results.
Methods: This is a prospective, open, randomized multicenter phase III clinical study with two arms that aims to evaluate the effects of pressurized intraperitoneal aerosol chemotherapy (PIPAC) combined with systemic chemotherapy vs. intravenous systemic chemotherapy alone on patients with metastatic upper GI tumors with a peritoneal seeding. Upper GI-adenocarcinomas originated from biliary tract, pancreas and stomach, or esophago- gastric junction are eligible. Patients in the study are treated with standard of care systemic palliative chemotherapy (mFOLFOX6) vs. PIPAC with intravenous (i.v.) chemotherapy (mFOLFOX6). Patients in first line with first diagnosed peritoneal seeding are eligible. Primary outcome is progression free survival (PFS).
Conclusions: PIPAC-procedure is explicit a palliative method but it delivers cytotoxic therapy like in hyperthermic intraperitoneal chemotherapy (HIPEC)-procedure directly to the tumor in a minimally invasive technique, without the need for consideration of the peritoneal-plasma barrier. The technique of PIPAC is minimally invasive and very gentle and the complete procedure takes only round about 45 min and, therefore, optimal in a clearly palliative situation where cure is not the goal. It is also ideal for using this approach in a first line situation, where deepest response should be achieved. The symbiosis of systemic therapy and potentially effective surgery has to be well-planned without deterioration of the patient due to aggressive way of surgery like in cytoreductive surgery (CRS)+HIPEC.

Background: Pancreatic ductal adenocarcinoma remains one of the most lethal malignancies, with few treatment options. NAPOLI 3 aimed to compare the efficacy and safety of NALIRIFOX versus nab-paclitaxel and gemcitabine as first-line therapy for metastatic pancreatic ductal adenocarcinoma (mPDAC).
Methods: NAPOLI 3 was a randomised, open-label, phase 3 study conducted at 187 community and academic sites in 18 countries worldwide across Europe, North America, South America, Asia, and Australia. Patients with mPDAC and Eastern Cooperative Oncology Group performance status score 0 or 1 were randomly assigned (1:1) to receive NALIRIFOX (liposomal irinotecan 50 mg/m2, oxaliplatin 60 mg/m2, leucovorin 400 mg/m2, and fluorouracil 2400 mg/m2, administered sequentially as a continuous intravenous infusion over 46 h) on days 1 and 15 of a 28-day cycle or nab-paclitaxel 125 mg/m2 and gemcitabine 1000 mg/m2, administered intravenously, on days 1, 8, and 15 of a 28-day cycle. Balanced block randomisation was stratified by geographical region, performance status, and liver metastases, managed through an interactive web response system. The primary endpoint was overall survival in the intention-to-treat population, evaluated when at least 543 events were observed across the two treatment groups. Safety was evaluated in all patients who received at least one dose of study treatment. This completed trial is registered with ClinicalTrials.gov, NCT04083235.
Findings: Between Feb 19, 2020 and Aug 17, 2021, 770 patients were randomly assigned (NALIRIFOX, 383; nab-paclitaxel–gemcitabine, 387; median follow-up 16·1 months [IQR 13·4–19·1]). Median overall survival was 11·1 months (95% CI 10·0–12·1) with NALIRIFOX versus 9·2 months (8·3–10·6) with nab-paclitaxel–gemcitabine (hazard ratio 0·83; 95% CI 0·70–0·99; p=0·036). Grade 3 or higher treatment-emergent adverse events occurred in 322 (87%) of 370 patients receiving NALIRIFOX and 326 (86%) of 379 patients receiving nab-paclitaxel–gemcitabine; treatment-related deaths occurred in six (2%) patients in the NALIRIFOX group and eight (2%) patients in the nab-paclitaxel–gemcitabine group.
Interpretation: Our findings support use of the NALIRIFOX regimen as a possible reference regimen for first-line treatment of mPDAC.
Einführung: Mehr als 75% der Cholecystektomien werden laparoskopisch operiert und weniger als 1/3 aller Gallenblasenkarzinome sind präoperativ bekannt. Seit den ersten Beschreibungen von Port-sitemetastasen 1991 und Tumorabsiedlungen 1994 wird vermutet, dass die laparoskopische Technik die Prognose von okkultem Gallenblasenkarzinom verschlechtern könnte. Material und Methode: 1997 hat die chirurgische Arbeitsgemeinschaft für Endoskopie (CAE) der deutschen Gesellschaft für Chirurgie ein Register gegründet, welches alle Fälle von postoperativen Zufallsbefund: „Gallenblasenkarzinom“ sammelt, egal ob die primäre Cholezystektomie laparoskopisch oder offen durchgeführt wurde. Zur Datengewinnung dient ein standardisierter Fragebogen, der an alle deutschen und nun auch an alle österreichischen chirurgischen Kliniken verschickt wurde, die Daten werden drei monatlich aktualisiert. Ziel des Registers ist es den Verlauf der Patienten zu beobachten und unter anderem die Frage zu beantworten, ob die Laparoskopie tatsächlich einen negativen Einfluß auf die Prognose des okkulten Gallenblasenkarzinoms hat. Außerdem wurde eine Internetseite zum Register vom Autor eingerichtet, die die neuesten Informationen zum Register und zum Krankheitsbild Gallenblasenkarzinom bietet, diese Seite ist Bestandteil der Doktorarbeit. Diese Seite ist unter der Adresse: www. ketteler-krankenhaus.de/register zu finden. Auch ist ein direktes Herunterladen des Fragebogens zum Register online möglich. Ergebnisse: 377 Fälle von okkultem Gallenblasenkarzinom sind bis dato von 186 Kliniken gemeldet worden. 201 Patienten wurden laparoskopisch operiert, 119 offen und bei 57 Patienten erfolgte ein intraoperativer Umstieg von laparoskopisch zu offen, aus nicht onkologischen Gründen. Es wurden uns 15 Port- site metastasen nach laparoskopischer Operation und 9 Wundmetastasen nach offener Operation gemeldet. Zu einem intraabdominellem Rezidiv kam es bei 23 laparoskopisch operierten Patienten und bei 13 primär offen operierten Patienten. Bei 123 der 377 Patienten wurde eine unmittelbare radikale Reoperation durchgeführt. Es scheint sich eine Tendenz in unserem Patientengut abzuzeichnen, dass reoperierte T1- und T2- Tumore von solch einem radikalen Zweiteingriff profitieren (log- rank > 0.05). Das kumulative Überleben nach Kaplan- Meier zeigt ein signifikant besseres Überleben für die laparoskopisch operierten Patienten im Vergleich zu den primär offen operierten (p=0,0127) oder den Patienten mit intraoperativem Umstieg (p=0,0191). Die 5 Jahresüberlebenswahrscheinlichkeit, (5 JÜR) für laparoskopisch und offen operierte T1- Tumore beträgt 55%, für laparoskopisch operierte T2- Tumore 40% und offen operierte T2- Tumore 30%. Der Gebrauch eines Bergebeutels scheint keinen positiven Effekt zu haben, die intraoperative akzidentelle Verletzung der Gallenblase hingegen scheint zu einer deutlichen Prognoseverschlechterung bezogen auf die Rezidivgenese in der laparoskopisch Gruppe zu haben. Diskussion: Die Inzidenz an Port-/ Wundmetastasen ist nahezu doppelt so hoch in der laparoskopischen Gruppe verglichen mit der offenen. Die Überlebensrate ist höher für die laparoskopisch operierten Patienten, was nicht damit erklärt werden kann, dass die Laparoskopie evtl. die weniger fortgeschritten Stadien operiert. Der Zugangsweg scheint also keinen wesentlichen Einfluß auf die Prognose des okkulten Gallenblasenkarzinoms zu haben. Nach der bisherigen Datenlage können wir keine Prognoseverschlechterung für die laparoskopisch operierten okkulten Gallenblasenkarzinome verifizieren.
Background and aims: Patients with gastric cancer often show signs of malnutrition. We sought to evaluate the influence of sarcopenia in patients with locally advanced, not metastasized, gastric or gastro-esophageal junction (GEJ) cancer undergoing curative treatment (perioperative chemotherapy and surgery) on morbidity and mortality in order to identify patients in need for nutritional intervention.
Patients and methods: Two-centre study, conducted in the Frankfurt University Clinic and Krankenhaus Nordwest (Frankfurt) as part of the University Cancer Center Frankfurt (UCT). 47/83 patients were treated in the FLOT trial (NCT01216644). Patients´ charts were reviewed for clinical data. Two consecutive CT scans were retrospectively analyzed to determine the degree of sarcopenia. Survival was calculated using the Kaplan-Meier method, multivariate analysis was performed using the Cox regression.
Results: 60 patients (72.3%) were male and 23 (27.7%) female. 45 patients (54.2%) had GEJ type 1–3 and 38 (45.8%) gastric tumors, respectively. Sarcopenic patients were significantly older than non-sarcopenic patients (mean age 65.1 years vs. 59.5 years, p = 0.042), terminated the chemotherapy significantly earlier (50% vs. 22.6%, p = 0.037) and showed higher Clavien-Dindo scores, indicating more severe perioperative complications (score ≥3 43.3 vs. 17.0%, p = 0.019). Sarcopenic patients had a significantly shorter survival than non-sarcopenic patients (139.6 ± 19.5 [95% CI, 101.3–177.9] vs. 206.7 ± 13.8 [95% CI, 179.5–233.8] weeks, p = 0.004). Multivariate Cox regression analysis showed that, besides UICC stage, sarcopenia significantly influenced survival.
Conclusion: Sarcopenia is present in a large proportion of patients with locally advanced gastric or GEJ cancer and significantly influences tolerability of chemotherapy, surgical complications and survival.
