Das Schilddrüsenkarzinom (SK) ist die häufigste bösartige endokrine Tumorerkrankung. Während das nicht-metastasierte und nicht-mutierte papilläre Schilddrüsenkarzinom (PSK) und das follikuläre Schilddrüsenkarzinom (FSK) eine gute Heilungschance aufweisen, zeigen die mutierten und metastasierten Varianten des PSK und FSK sowie das anaplastische Schilddrüsenkarzinom (ASK) weiterhin eine schlechte Prognose. Die Entwicklung von Therapieresistenzen stellen hierbei ein Hauptproblem in der Behandlung des fortgeschrittenen Schilddrüsenkarzinoms dar.
In den letzten Jahren wurden in Studien zunehmend Tumor-initiierende Zellen (TIZ) beschrieben, welche eine kleine Subpopulation von Zellen mit der Fähigkeit zur Selbsterneuerung, Tumorinitiierung und Entwicklung von Therapieresistenzen von Tumoren darstellen. Die Existenz von TIZ wurde auch im SK nachgewiesen. Ein entscheidender Faktor für die Persistenz von TIZ ist die Hypoxie, welche über eine Veränderung des Tumormikromilieus und des Zellmetabolismus zur Entstehung von Therapieresistenzen beiträgt. Ein durch Hypoxie hochreguliertes Enzym ist die Carboanhydrase IX (CAIX). CAIX wird hauptsächlich von Tumorzellen exprimiert und katalysiert die Reaktion von Kohlendioxid zu Bicarbonat und einem Proton und trägt damit zur Säurepufferung der Tumorzelle bei. CAIX stellt somit einen entscheidenden Faktor für das Überleben von Tumorzellen in einem hypoxischen Milieu dar. Des Weiteren ist eine erhöhte Expression von CAIX mit einem schlechten Patienten-Outcome assoziiert, wie z.B. im Brustkrebs. Diese Eigenschaften machen CAIX zu einem attraktiven Angriffspunkt einer zielgerichteten Tumortherapie. Die vorliegende Studie hat zum Ziel, die Expression von CAIX sowie dessen biologische Rolle im Schilddrüsenkarzinom näher zu untersuchen.
Hierzu wurden Proben von 114 SK-Patienten immunhistochemisch auf eine CAIX-Expression untersucht und mit tumorfreiem Schilddrüsengewebe verglichen. Hierbei waren unterschiedliche SK-Subtypen vertreten. Für eine weitere Validierung der Expressionsdaten erfolgte die Auswertung eines Datasets von „The Cancer Genome Atlas“ (TCGA) mithilfe von cBioportal. Da die Hypoxie ein wichtiger Faktor für die Persistenz von TIZ ist, wurde die CAIX-Expression in Tumorsphären, ein in vitro Nachweis von TIZ-Aktivität, mittels der Durchflusszytometrie bestimmt und mit der CAIX-Expression von Monolayern verglichen. Als SK-Zelllinien wurden BCPAP (PSK), FTC 133 (FSK) und 8505 C (ASK) verwendet. Anschließend wurde mithilfe der Polymerasekettenreaktion und Immunofluoreszenzfärbung untersucht, ob eine CAIX-Expression in den Tumorsphären mit der Expression von bereits bekannten Stammzellmarkern, u.a. NANOG, assoziiert ist. Die Unterschiede der CAIX-Expression, nach Inkubation der Monolayer jeweils in Normoxie und Hypoxie, wurden mittels Durchflusszytometrie bestimmt. Mithilfe eines genetischen CAIX-Knockdowns sowie einer pharmakologischen Inhibition mit dem CAIX-Inhibitor Methazolamid (MZM) wurde die Tumorzellproliferation und -Sphärenbildung unter Normoxie und Hypoxie bestimmt. Zusätzlich wurde der Einfluss von MZM auf die Apoptose und den Zellzyklus untersucht.
Immunhistochemische Färbungen der Gewebeproben von SK-Patienten zeigten, dass die CAIX-Expression sowohl im PSK und FSK als auch im ASK im Vergleich zum tumorfreien Schilddrüsengewebe erhöht war. Des Weiteren zeigte die klinisch-pathologische Datenanalyse, dass eine erhöhte CAIX-Expression mit dem Auftreten von Lymphknotenmetastasen im differenzierten SK assoziiert war. Auch die Analyse des TCGA-Datasets bestätigte, dass eine erhöhte Expression der CAIX-mRNA mit einem fortgeschrittenen Tumorstadium, Fernmetastasen und mit einem kürzeren Gesamt-Überleben von SK-Patienten korrelierte. Die weiteren funktionellen in vitro Untersuchungen ergaben, dass die CAIX-Expression in den Tumorsphären im Vergleich zu Monolayern erhöht und mit einer erhöhten Expression von Stammzellmarkern assoziiert war. Ein genetischer CAIX-Knockdown und eine CAIX-Inhibition mit MZM führten über eine Induktion der Apoptose und eines Zellzyklusarrests zu einer verminderten Tumorzellproliferation und Sphärenbildung.
Zusammenfassend deuten die Ergebnisse darauf hin, dass CAIX ein vielversprechendes Zielmolekül für eine gezielte Tumortherapie des fortgeschrittenen SK ist. Um diese Hypothese bestätigen zu können, sind jedoch weitere prospektive Analysen von Patientenproben sowie funktionelle in vivo Untersuchungen am SK nötig.
Einleitung: Die Zentrale Notaufnahme (ZNA) stellt eine Schnittstelle zwischen prä- und innerklinischer Versorgung dar. Das Spektrum der Krankheitsbilder erstreckt sich über jegliche Fachrichtungen und variiert von harmlosen Banalitäten bis zu akuten Notfällen. Eine sichere und suffiziente Primärversorgung ist die Basis eines qualitativ-hochwertigen Gesundheitssystems.2 Verspätete oder falsche Diagnosen in der ZNA sind mit 10-30 % keine Seltenheit.
Dies ist nicht nur für den individuellen Patienten belastend, es bedeutet auch einen zusätzlichen Ressourcenverbrauch und eine finanzielle Belastung für das Gesundheitssystem. Clinical Decision Support Systems (CDSS) haben das Potenzial, sowohl professionelle Anwender als auch Laien bei ihrer Diagnosefindung zu unterstützen. Fragestellung: Ziel der Arbeit ist es herauszufinden, welchen Einfluss der Diagnosezeitpunkt auf das Outcome von Patienten mit Abdominalschmerzen in der ZNA hat und inwiefern ein CDSS das Potenzial hat, die genannten Punkte zu beeinflussen.
Methoden: Es handelte sich um eine prospektive, doppelt verblindete Beobachtungsstudie. Patienten, die sich mit Abdominalschmerzen in der Notaufnahme vorstellten, gaben ihre Symptome in die Ada-App ein. Sowohl die Diagnosevorschläge der App als auch die Verdachtsdiagnosen des behandelnden Arztes wurden dokumentiert und verglichen. Weitere erhobene Parameter waren die Verwendung von apparativer Diagnostik, die vergangene Zeit bis zur endgültigen Diagnosestellung, das Auftreten von Komplikationen, die Komorbidität und Mortalität sowie die Länge des Krankenhausaufenthalts. Das Follow-Up erfolgte zu verschiedenen Zeitpunkten bis zu Tag 90. Für die Analyse wurden die 450 Patienten anhand des Zeitpunkts ihrer Diagnosestellung in Gruppen "früh" (Tag 0) und "spät" (Tag 1-24) eingeteilt.
Ergebnisse: Im Vergleich zur „frühen“ Gruppe, hatte die Gruppe der „spät“ diagnostizierten Patienten einen höheren Anteil von Männern (45.2% (n=168/372) versus 60.3 % (n=47/78); p=0.018), im Schnitt einen höheren Charlson Comorbidity Index (0.7 versus 1.1; p=0.045) und im Schnitt einen höheren RAI-C Score (8.06 versus 9.9; p<0.001) am Tag ihrer Vorstellung. Bei den „spät“ diagnostizierten blieben weniger Patienten komplikationsfrei (57.0 % 49 (n=212/372) versus 17.9% (n=14/78); p<0.001), es traten mehr Major-Komplikationen auf (8.9% (n=33/372) versus 17.9% (n=14/78); p=0.024), analog dazu war der Comprehensive Complication Index höher (13.1 versus 25.6; p<0.001) und sie verweilten länger im Krankenhaus (2.6 Tage versus 6.7 Tage; p<0.001). Zudem nahmen sie signifikant mehr apparative Diagnostik in Anspruch.
Die behandelnden Ärzte konnten in 82.6% der Fälle (n=372/450) am Tag der Vorstellung die korrekte Diagnose stellen. Die Ada-App konnte in insgesamt 52% der Fälle (n=234/450) die korrekte Diagnose unter ihren Diagnoseverschlägen nennen.
Schlussfolgerung: Multimorbide Patienten scheinen anfälliger zu sein für falsche und verspätete Diagnosen. Ein später Diagnosezeitpunkt korreliert mit der vermehrten Nutzung apparativer Diagnostik, einem komplikationsreicheren Verlauf, einer höheren Komorbidität und einem längeren Krankenhausaufenthalt.
Die Ada-App ist den Ärzten zwar unterlegen, dennoch ist Potenzial vorhanden.
Für Ärzte kann die Ada-App eine Unterstützung im Rahmen der Diagnosefindung darstellen. Neben der Ressourcenentlastung kann sie vor Fehlannahmen und frühzeitigen Schlussfolgerungen schützen und auf weitere mögliche Differentialdiagnosen hinweisen. Die Ada-App stellt für Laien mit Sicherheit eine Weiterentwicklung gegenüber der simplen Symptomsuche über das Internet dar, dennoch sollten weitere Studien den Nutzen und die Sicherheit der App überprüfen.
Weltweit gibt es laut WHO ca. 58 Millionen Menschen mit einer chronischen Hepatitis-C-Virus (HCV) Infektion und jährlich stecken sich ca. 1,5 Millionen Menschen neu mit diesem Virus an (Stand 2019). Da die Folge einer chronischen Hepatitis-C-Virus Infektion eine potenziell tödlich verlaufende Leberzirrhose oder die Entwicklung eines Hepatozellulären Karzinoms sein können, ist eine frühe Diagnose und eine adäquate Therapie eine wichtige Aufgabe in der Medizin.
Die bisherige Therapie erfolgte mittels pegyliertem Interferon und Ribavirin und seit einigen Jahren auch interferonfrei mittels Direkter Antiviraler Agenzien (DAA). Vor allem beim älteren Therapieregime konnten viele Nebenwirkungen und häufiger auch ein Therapieversagen auftreten, sodass ein leicht zu gewinnender Biomarker nützlich wäre, der die Patienten mit Therapieversagen frühzeitig und im besten Fall sogar vor Therapiebeginn detektieren kann.
In der vorliegenden Arbeit wurden die Spiegel von extrazellulär im Blut zirkulierender, leberspezifischer microRNA miR-122 auf Eigenschaften als solche potenzielle Biomarker untersucht. Dazu wurden die Patientenseren von insgesamt 60 Patienten mit chronischer Hepatitis-C-Virus Infektion analysiert, die mittels pegyliertem Interferon und Ribavirin behandelt wurden. Vor, während und nach der Therapie wurden verschiedene Laborparameter sowie die miR 122 in den Patientenseren bestimmt. 20 dieser Patienten zeigten ein dauerhaftes Ansprechen auf die Therapie (sustained virological response = SVR), 20 zeigten nach einem initialen Therapieansprechen ein Rückfall der Erkrankung (Relapse) und 20 Patienten sprachen gar nicht auf die Therapie an (Non-Responder = NR).
Zunächst wurden die sogenannten Baseline-Charakteristika der Patienten vor Therapiebeginn untersucht. Dabei konnten wir jedoch keinen Unterschied zwischen den Patientengruppen hinsichtlich der Alanin-Aminotransferase (ALT) und Aspartat-Aminotransferase (AST), zwei Laborparameter zur Bestimmung einer Leberschädigung, sowie der HCV-RNA, ein Parameter zur Bestimmung der Viruslast bei Patienten mit einer HCV-Infektion, feststellen.
Auch die miR-122-Spiegel zeigten vor Therapiebeginn keinen signifikanten Unterschied zwischen den drei Patientengruppen. Daraus wurde geschlossen, dass man die miR-122 vor Therapiebeginn nicht als prognostischen Marker für einen Therapieerfolg verwenden kann.
Beim Vergleich der miR-122-Spiegel mit den Laborparametern konnte eine signifikante Korrelation zwischen der miR-122 und der ALT, AST und der Gamma-Glutamyl-Transferase (GGT) festgestellt werden. Die miR-122 scheint somit ähnlich wie die anderen Laborparameter eine Leberzellschädigung widerzuspiegeln.
Nach dem Therapiebeginn konnte bereits ab Woche 4 ein signifikanter Unterschied zwischen den SVR und Non-Respondern sowie zwischen den Relapsepatienten und den Non-Respondern festgestellt werden. Jedoch war der Unterschied zwischen den SVR und den Relapse-Patienten nicht signifikant, sodass man weiterhin keine Unterscheidung dieser beiden Patientengruppen machen konnte.
Auch die ALT- und HCV-RNA-Spiegel zeigten einen ähnlichen Verlauf. In den Gruppen der SVR und Relapse-Patienten zeigte sich im Laufe der Therapie ein Rückgang der Parameter wohingegen die Gruppe der Non-Responder keine signifikante Dynamik aufwies.
Zum Schluss wurden die miR-122-Spiegel 12 bzw. 24 Wochen nach Therapieende bestimmt, dem sogenannten Zeitpunkt des Follow-Up bei dem der Therapieerfolg laut Leitlinie mit Hilfe der HCV-RNA-Messung bestimmt wird. Dabei konnte ein signifikanter Unterschied zwischen den miR-122-Spiegeln bei den SVR-Patienten und den anderen beiden Patientengruppen festgestellt werden.
In Zusammenschau dieser Ergebnisse kann man sagen, dass die miR-122 gut geeignet ist um ähnlich wie die HCV-RNA den Therapieverlauf widerzuspiegeln. Als prognostischer Parameter bzw. Biomarker für ein Therapieansprechen ist sie jedoch nicht geeignet, da keine Unterscheidung zwischen den einzelnen Patientengruppen vor Therapiebeginn möglich ist und während der Therapie lediglich die Non-Responder und nicht die Relapse-Patienten detektiert werden können.
Im Fokus der vorliegenden Arbeit stand die Fragestellung, inwieweit ORF10 von B. recurrentis, dem Erreger des Läuserückfallfiebers, mit verschiedenen Komplementkomponenten interagieren kann.
Mit funktionellen Komplementtests konnte gezeigt werden, dass ORF10 den klassischen, den alternativen sowie den Lektin-Weg inhibiert, wobei sich die stärkste Inhibition gegenüber dem alternativen Weg manifestierte. Darüber hinaus ließ sich in einem Zell-basierten Hämolyse-Assay eine Inhibition des terminalen Komplementweges durch ORF10 nachweisen.
Die durchgeführten Bindungsanalysen mit verschiedenen Komplementkomponenten führten zu dem Ergebnis, dass ORF10 mit C1q, C1s, C3, C3b, C4b und C5 interagiert und diese Protein-Protein-Interaktion mit den Komplementkomponenten C1q, C3, C3b und C4b durch einen dosisabhängigen Verlauf charakterisiert ist. Eine Bindung von C1r, C2, C4, FB, FH sowie FI konnte jedoch nicht nachgewiesen werden.
Auf Basis der modellierten Struktur von ORF10 wurden mithilfe der In-vitro-Mutagenese drei verschiedene Varianten generiert, die jedoch nicht affinitätschromatographisch aus E. coli-Zelllysaten isoliert werden konnten und deshalb nicht für weiterführende funktionelle Analysen zur Verfügung standen.
Die Ergebnisse der Serumbakterizidie-Tests mit einem „gain of function“-Borrelienstamm, welcher ORF10 heterolog produzieren sollte, ergaben, dass keine erhöhte Resistenz gegenüber Humanserum nachgewiesen werden konnte. Inwieweit ORF10 bei der intrinsischen Serumresistenz von B. recurrentis beteiligt ist, lässt sich abschließend nicht vollumfänglich erklären.
Background: Survival data regarding cytoreductive nephrectomy (CN) in metastatic renal cell carcinoma (mRCC) patients according to the type and extent of tumor-associated vascular thrombus are scarce.
Objective: To test for survival differences in mRCC patients treated with CN according to the type and extent of tumor-associated vascular thrombus.
Design, setting, and participants: Within Surveillance, Epidemiology, and End Results Research Plus (2004–2017), we identified CN mRCC patients with renal vein (pT3a-TT) versus infradiaphragmatic inferior vena cava (IVC; pT3b) versus supradiaphragmatic IVC tumor thrombus/IVC invasion (pT3c).
Outcome measurements and statistical analysis: Overall survival (OS) was addressed in Kaplan-Meier and Cox regression analyses, in addition to 3-mo landmark analyses.
Results and limitations: Of 2170 mRCC patients, 1880 (87%), 204 (9%), and 86 (4%) harbored pT3a-TT, pT3b, and pT3c, respectively. The respective median OS periods were 21, 23, and 12 mo (p < 0.001). In multivariable Cox regression models, pT3c stage, but not pT3b stage, was an independent predictor of higher overall mortality (hazard ratio [HR]: 1.37; 95% confidence interval [CI]: 1.09–1.73; p = 0.007), as well as in 6-mo landmark analyses (HR: 1.36; 95% CI: 1.02–1.80; p = 0.04). In the sensitivity analysis, relying on all pT3a patients, the predictor status of pT3c stage remained unchanged (HR: 1.37; 95% CI: 1.09–1.71; p = 0.007). Limitations have to be addressed regarding the sample size and the retrospective design of the current study.
Conclusions: Although overall mortality is significantly higher in pT3c mRCC patients than in their pT3b and pT3a-TT counterparts, these individuals may still expect 12-mo or better OS after CN versus virtually 2-yr OS in their pT3a and pT3b counterparts.
Patient summary: In this study, we looked at the survival outcomes of metastatic renal cell carcinoma patients who presented with tumor thrombus at cytoreductive nephrectomy. Even though these patients with most advanced tumor thrombus stage demonstrated lower survival rates, the median overall survival was still 1 yr.
Middle-aged persons with multimorbidity have to take their illnesses into account in their daily work, family and leisure activities. The MuMiA project aims to identify early preventive measures that make it easier for those between 30 and 60 years of age with multiple chronic diseases to manage their illnesses in their everyday lives. An interdisciplinary workshop and interviews with multimorbid middle-aged adults and their principal healthcare providers will be used to collect information on the management of care in the contexts of patients’ daily work, family and leisure activities. Data obtained in the interviews will be coded inductively and analysed using content analysis. Workshop outputs will be transcribed and evaluated by the authors. This study has received ethical approval from the Faculty of Medicine Ethics Committee of Goethe University (2021-47). The project will generate prevention recommendations that reflect the experiences of middle-aged persons living with multimorbidity and the views of their principal healthcare providers. The findings will be disseminated via conferences and peer-reviewed publications.
Development of a multidimensional pain questionnaire in professional dance (MPQDA): a pilot study

Background: Pain is part of the everyday life of professional dancers. It can indicate health risks and impair the ability to work. Suitable screening tools can be used to identify pain and its risk potential. A comprehensive, multidimensional, differentiated assessment tool for pain in professional dance does not currently exist.
Methods: An initial questionnaire was developed in German and English and was assessed in a qualitative pretest. In a field study with a cross-sectional design including n = 72 dancers from Germany (n = 36 responses each in the English and German language versions), the questionnaire was optimized by item analysis, its psychometric properties (dimensionality, construct validity, reliability) were examined and the ability of the pain dimensions to classify the subjective ability to work in training was analyzed (ROC analysis).
Results: The developed Multidimensional Pain Questionnaire in Professional Dance (MPQDA) was reduced and optimized in its psychometric properties. Following questions were reduced in their items or answer categories: pain localizations (from 20 to 15 regions), accompanying symptoms (from 6 to 3 items), sensory and affective pain quality (from 20 to 10 items), pain frequency (from 4 to 3 answer categories), and the motives of working with pain (from 14 to 12 items). Regarding the subjective ability to work in training, the variables of the ability to work in rehearsals and in performances, as well as the accompanying symptoms of tension and mobility restrictions, showed a relatively good classification ability (Area under the Curve (AUC) ≥ 0.7 in the 95% confidence interval) and significant, moderate to strong correlations (Somers' D > 0.25, p < 0.05). The classification ability of the other pain dimensions was largely absent or poor.
Conclusion: The MPQDA differentiates various pain dimensions in professional dancers and is available in a compatible manner in German and English. The clinical relevance needs to be explored further in the future.
Up to the present, there has been a lack of studies on the skin health of professional and recreational dancers. Dancers are at risk of skin diseases due to contact with allergenic or irritating substances and working in humid environments. The aim of the present study was, therefore, to examine skin health in two different dance styles and training periods. Methods: Physical dermatological examination of professional dancers (PD; n = 35) and Latin American formation dancers (LD; n = 79) after a 4-week period of recovery (T0) and a period of high training or work load (T1). Results: PD are significantly more frequently affected by skin dermatoses than LD (T0, p = 0.004) (frontal traction alopecia, hair loss, facial seborrhoea, xerosis cutis of the trunk and extremities, and facial folliculitis). The following significant differences between the sexes were observed in the LD: more folliculitis of the trunk in male subjects (T0 and T1, p = 0.009), more frequent xerosis cutis of the extremities (p < 0.001) and perioral dermatitis in female subjects (T1, p = 0.043). Subjects with skin lesions trained more frequently, performed more times per year, and had longer dance experience. Discussion: Based on the findings, preventive measures for skin protection (especially informing dancers about skin health) are necessary. At the same time, further studies on this topic are important.
The deep fascia is intimately linked to skeletal muscle and may be involved in delayed onset muscle soreness (DOMS). The present study therefore explored the effect of eccentric exercise on fascia stiffness and its relation with DOMS. Healthy active male adults (n = 19, 27 ± 4 years) performed 6 x 10 maximal eccentric knee flexions using an isokinetic dynamometer. Before (baseline) as well as immediately (T0), 1 hour (T1), and each day up to 72 hours (T24 to T72) afterwards, shear wave elastography was used to measure the mechanical stiffness of the biceps femoris muscle and the overlying fascia. As a surrogate of DOMS, pain upon palpation was captured by means of a 100mm visual analogue scale. While muscle stiffness remained unchanged (p > 0.05), deep fascia stiffness increased from baseline to T24 (median: 18 kPa to 21.12 kPa, p = 0.017) and T72 (median: 18 kPa to 21.3 kPa, p = 0.001) post-exercise. Linear regression showed an association of stiffness changes at T24 and pressure pain at T72 (r2 = 0.22, p < 0.05). Maximal eccentric exercise leads to a stiffening of the fascia, which, in turn, is related to the magnitude of future DOMS. Upcoming research should therefore gauge the effectiveness of interventions modifying the mechanical properties of the connective tissue in order to accelerate recovery.
Background: Primary viral myocarditis associated with severe acute respiratory syndrome coronavirus 2 (SARS-Cov2) infection is a rare diagnosis.
Case presentation: We report the case of an unvaccinated, healthy patient with cardiogenic shock in the context of a COVID-19-associated myocarditis and therapy with simultaneous veno-arterial extracorporeal membrane oxygenation (VA-ECMO) and percutaneous left ventricular decompression therapy with an Impella. The aim of this review is to provide an overview of therapeutic options for patients with COVID-19-associated myocarditis.
Conclusions: The majority of patients required a combination of two assist devices to achieve sufficient cardiac output until recovery of left ventricular ejection fraction. Due to the rapid onset of this fulminant cardiogenic shock immediate invasive bridging therapy in a specialized center was lifesaving.
Introduction: Studies of vocational ballet students are sparce. In particular, there is a lack of gender comparisons. The aim of the present study, therefore, was to give a musculoskeletal and sociodemographic description of the typical vocational ballet student in gender comparison. Methods: In this study, n = 414 female and n = 192 male students of the John Cranko School (JCS), aged between 5 and 22 years (Mean ± SD: 13.9 ± 3.5), were examined by an experienced orthopedist and dance physician. Results: Males started ballet (5.8/8.2 years, p < 0.001) and training at later age than females (13.5/14.6 years, p < 0.05). There was a high prevalence of low body weight among both sexes; however, particularly among female participants (58.4/16.2%, p < 0.001). Both sexes showed a large external rotation of the hip (f/m: 59/62°, p < 0.001), a large turnout (f/m: 82/86°, p < 0.01), high values for plantarflexion of the ankle joint (f/m: 72/68°, p < 0.001) and dorsiflexion of the metatarsophalangeal joint of the big toe (f/m: 90/87°, p < 0.001). Discussion: Differences in ballet-specific characteristics between genders (f/m) are converging and are smaller than described in the past. The particularly high prevalence of low body weight among students in the vocational training sector, particularly among females, highlights the need for deeper diagnostic investigation.
Cortical tracking of stimulus features (such as the envelope) is a crucial tractable neural mechanism, allowing us to investigate how we process continuous music. We here tested whether cortical and behavioural tracking of beat, typically related to rhythm processing, are modulated by pitch predictability. In two experiments (n=20, n=52), participants’ ability to tap along to the beat of musical sequences was measured for tonal (high pitch predictability) and atonal (low pitch predictability) music. In Experiment 1, we additionally measured participants’ EEG and analysed cortical tracking of the acoustic envelope and of pitch surprisal (using IDyOM). In both experiments, finger-tapping performance was better in the tonal than the atonal condition, indicating a positive effect of pitch predictability on behavioural rhythm processing. Neural data revealed that the acoustic envelope was tracked stronger while listening to atonal than tonal music, potentially reflecting listeners’ violated pitch expectations. Our findings show that cortical envelope tracking, beyond reflecting musical rhythm processing, is modulated by pitch predictability (as well as musical expertise and enjoyment). Stronger cortical surprisal tracking was linked to overall worse envelope tracking, and worse finger-tapping performance for atonal music. Specifically, the low pitch predictability in atonal music seems to draw attentional resources resulting in a reduced ability to follow the rhythm behaviourally. Overall, cortical envelope and surprisal tracking were differentially related to behaviour in tonal and atonal music, likely reflecting differential processing under conditions of high and low predictability. Taken together, our results show diverse effects of pitch predictability on musical rhythm processing.
Structural brain morphometry as classifier and predictor of ADHD and reward-related comorbidities

Attention deficit/hyperactivity disorder (ADHD) is one of the most common neurodevelopmental disorders, and around two-thirds of affected children report persisting problems in adulthood. This negative trajectory is associated with high comorbidity with disorders like obesity, depression, or substance use disorder (SUD). Decreases in cortical volume and thickness have also been reported in depression, SUD, and obesity, but it is unclear whether structural brain alterations represent unique disorder-specific profiles. A transdiagnostic exploration of ADHD and typical comorbid disorders could help to understand whether specific morphometric brain changes are due to ADHD or, alternatively, to the comorbid disorders. In the current study, we studied the brain morphometry of 136 subjects with ADHD with and without comorbid depression, SUD, and obesity to test whether there are unique or common brain alterations. We employed a machine-learning-algorithm trained to classify subjects with ADHD in the large ENIGMA-ADHD dataset and used it to predict the diagnostic status of subjects with ADHD and/or comorbidities. The parcellation analysis demonstrated decreased cortical thickness in medial prefrontal areas that was associated with presence of any comorbidity. However, these results did not survive correction for multiple comparisons. Similarly, the machine learning analysis indicated that the predictive algorithm grouped most of our ADHD participants as belonging to the ADHD-group, but no systematic differences between comorbidity status came up. In sum, neither a classical comparison of segmented structural brain metrics nor an ML model based on the ADHD ENIGMA data differentiate between ADHD with and without comorbidities. As the ML model is based in part on adolescent brains, this might indicate that comorbid disorders and their brain changes are not captured by the ML model because it represents a different developmental brain trajectory.
Objectives: The range of motion (ROM) of the cervical spine and postural stability are important for an economical and motorically adequate adaptation of the body to any situation. Therefore, this study aims to analyze whether these two components of postural and movement control can be influenced by means of a splint in a centric position compared to habitual occlusion.
Methods: 38 recreational male athletes volunteered. Cervical spine ROM was recorded using an ultrasound system and the a pressure measuring plate for postural stability (length of center of pressure (CoP) movement, area of CoP). The two dental occlusion conditions employed were the habitual occlusion and wearing a splint in an idealized, condylar position close to the centric position. Level of significance was set at ρ ≤ 0.05.
Results: The cervical spine mobility increased significantly by wearing the splint regarding rotation to the left (+3.9%) and right (+2.7%) and lateral flexion to the left (+4.4%) and right (+6.7%). Wearing the splint reduced the area of sway deflections by about 31.5% in the bipedal stance and by about 2.4% (left) and 28.2% (right) in the unipedal stance. The CoP trace was reduced in the sagittal plane by approximately 8.2% in the right single-leg stance.
Conclusions: The major findings seem to demonstrate that wearing a splint that keeps the jaw close to the centric relation may increase the cervical ROM and may improve balance stability in male recreational athletes. Changing the jaw relation in athletes can possibly aid the release of performance potentials by improving coordination skills.
Background: In order to determine possible pathological deviations in body weight distribution and body sway, it is helpful to have reference values for comparison: gender and age are two main influencing factors. For this reason, it was the aim of the present study to present reference values for women between 51 and 60 years of age.
Methods: For this study, 101 subjectively healthy female Germans aged between 51 and 60 years (55.16 ± 2.89 years) volunteered and were required to stand in a habitual posture on a pressure measuring platform.
Results: The average BMI of this age group was 25.02 ± 4.55 kg/m². The left and right foot showed an almost evenly balanced load distribution with a median load of 52.33% on the left foot [tolerance interval (TR) 38.00%/68.03%; confidence interval (CI) 51.00%/53.33%] and 47.67% on the right foot [TR 31.97%/62.00%; CI 46.67%/49.00%]. The measured median load of the forefoot was 33.33% [TR 21.37%/54.60%; CI 30.67%/36.00%] and that of the rear foot was 66.67% [TR 45.50%/78.63%; CI 64.00%/69.33%]. The median body sway in the frontal plane was 11 mm [TR 5.70 mm/26.30 mm; CI 10.00 mm/11.67 mm] and that of the sagittal plane was 16 mm [TR 7.37 mm/34.32 mm; CI 14.67 mm/18.67 mm]. The median ellipse area was 1.17 cm² [TR 0.29 cm²/4.96 cm²; CI 0.98 cm²/1.35 cm²], the median ellipse width was 0.91 cm [TR 0.42 cm/1.9 cm; CI 0.84 cm/1.02 cm] and its height was 0.40 cm [TR 0.22 cm/0.89 cm; CI 0.38 cm/0.43 cm].
Conclusions: The left-to-right ratio is almost balanced. The load distribution of the forefoot to the rear foot is approximately 1:2. The median body sway values for the frontal and sagittal planes (11 and 16 mm, respectively) agree with other values. The values for the height, body weight and the BMI are comparable to the values of average German women at this age; therefore, the measured values show a presentable cross section of women in the 51–60 age group in Germany. The present data can be used as a basis for women aged 51–60 years and can support the detection of possible dysfunctions as well as injury prevention in the parameters of postural control.
Background: In the COVID-19 pandemic, numerous researchers postponed their patient and public involvement (PPI) activities. This was mainly due to assumptions on patients’ willingness and skills to participate digitally. In fact, digital PPI workshops differ from in-person meetings as some forms of non-verbal cues and body language may be missing and technical barriers may exist. Within our project HYPERION-TransCare we adapted our PPI workshop series for intervention development to a digital format and assessed whether these digital workshops were feasible for patients, health care professionals and researchers.
Methods: We used a digital meeting tool that included communication via audio, video and chat. Discussions were documented simultaneously on a digital white board. Technical support was provided via phone and chat during the workshops and with a technical introduction workshop in advance. The workshop evaluation encompassed observation protocols, participants’ feedback via chat after each workshop on their chance to speak and the usability of the digital tools, and telephone interviews on patients’ and health professionals’ experiences after the end of the workshop series.
Results: Observation protocols showed an active role of moderators in verbally encouraging every participant to get involved. Technical challenges occurred, but were in most cases immediately addressed and solved. Participants median rating of their chance to speak and the usability of the digital tool was “very good”. In the evaluation interviews participants reported a change of perspective and mutual understanding as a main benefit from the PPI workshops and described the atmosphere as inclusive and on equal footing. Benefits of the digital format such as overcoming geographical distance, saving time and combining workshop participation with professional or childcare obligations were reported. Technical support was stressed as a pre-condition for getting actively involved in digital PPI.
Conclusions: Digital formats using different didactic and documentation techniques, accompanied by technical support, can foster active patient and public involvement. The advantages of digital PPI formats such as geographical flexibility and saving time for participants as well as the opportunity to prepare and hold workshops in geographically stretched research teams persists beyond the pandemic and may in some cases outweigh the advantages of in-person communication.
Although, during the past decades, substantial advances emerged in identifying major local and systemic factors contributing to initiation and progression of osteoarthritis (OA), some neuroendocrine mechanisms are still not understood or even neglected when thinking about novel therapeutic options. One of which is the sympathetic nervous system that exhibits various OA-promoting effects in different tissues of the joint. Interestingly, the β2-adrenoceptor (AR) mediates the majority of these effects as demonstrated by several in vitro, in vivo as well as in clinical studies. This review article does not only summarize studies of the past two decades demonstrating that the β2-AR plays an OA-promoting role in different tissues of the joint but also aims to encourage the reader to think about next-level research to discover novel and innovative preventive and/or therapeutic strategies targeting the β2-AR in OA.
Das Hepatoblastom ist ein embryonaler Lebertumor, dessen Zellen unterschiedliche unreife Stadien aufweisen. Aufgrund dieser Unreife ist die Identifizierung von Krebsstammzellen erschwert, die in diesem Tumor vermutet und für Rückfälle verantwortlich gemacht werden. In Vorarbeiten konnte eine Krebsstammzellpopulation mit der Kombination der hämatopoetischen Stammzellmarker CD90 und CD34 sowie dem „oval cell“ OV-6-Antikörper in den etablierten Hepatoblastomzelllinien HuH6 und HepG2 detektiert werden. Diese CD34+OV-6+CD90+ Zellen wiesen eine erhöhte Expression von Pluripotenzfaktoren auf und zeichneten sich durch ein erhöhtes Migrationsverhalten aus.
In der hier vorgelegten Arbeit wurden zunächst Tumor-Sphäroid-Assays durchgeführt um diese Population als Krebsstammzellen zu bestätigen, da nur Krebsstammzellen unter den gegebenen Umständen wachsen können. Diese waren im Anschluss wieder in der Lage, in „normalem Medium“ zu differenzieren. Des Weiteren wurden die Zelllinien mit dem Standardtherapeutikum Cisplatin behandelt. Da Krebsstammzellen als chemoresistent gelten, konnte in der überlebenden Zellpopulation auch eine Anreicherung der CD34+OV-6+CD90+ Zellen beobachtet werden. Zusätzlich ließ sich eine weitere CD34+OV-6+CD90+ Population identifizieren, deren Expression aller drei Marker schwächer war und die bei steigenden Cisplatin-Konzentrationen den Großteil der CD34+OV-6+CD90+ Population ausmachte.
Neben den erhöhten Transkriptmengen der Pluripotenzmarker Oct4 und Nanog zeichneten sich die CD34+OV-6+CD90+ Krebsstammzellen durch eine verstärkte Expression der aktivierungsinduzierten Zytidin-Desaminase AID aus. AID wirkt induzierend auf die Transkription beider Pluripotenzmarker. Daher könnte es auch bei Krebsstammzellen eine Rolle bei der Induktion von Pluripotenz und bei ihrer langfristigen Erhaltung spielen. Dies unterstützend legten die verminderten Transkriptmengen der Pluripotenzgene nach einem AID siRNA Knock-Down eine Abhängigkeit des Stammzellcharakters von diesem Faktor nahe. Um einen Effekt von Therapeutika, die inhibitorisch auf AID wirken, auf Krebsstammzellen zu untersuchen, wurden die Zellen mit den DNMT-Inhibitoren Decitabine und Zebularine sowie dem HSP90-Inhibitor Tanespimycin behandelt. Tatsächlich konnte vor allem in den HuH6-Zellen ein verminderter Anteil der Krebsstammzellpopulation durch die drei Substanzen beobachtet werden. Wurden die Zellen im Anschluss mit dem Standardzytostatikum Cisplatin behandelt, führte allerdings eine Vorbehandlung mit Zebularine oder Decitabine zu einer starken Anreicherung von Krebsstammzellen. Dies war vor allem auf Zellen mit dem schwach positiven CD34+OV-6+CD90+ Expressionsprofil zurückzuführen, die chemo-induziert zu sein scheinen. Zwar erscheint die Mehrheit der Tumorzellen eliminiert zu werden, jedoch lassen diese Ergebnisse die Entstehung von chemo-induzierten Krebsstammzellen vermuten, die langfristig für einen Rückfall verantwortlich sein könnten. Daher ist von einer Ergänzung der Therapie mit diesen Substanzen abzusehen. Eine Vorbehandlung mit Tanespimycin hingegen konnte im Vergleich zur alleinigen Cisplatin-Behandlung wirksam den Anteil der Krebsstammzellen reduzieren. Interessanterweise war dies nicht auf einen verstärkten Zelltod der Krebsstammzellen, sondern vielmehr auf eine durch Tanespimycin bewirkte Differenzierung derselben zurückzuführen. Diese spiegelte sich auch in einer verminderten Expression von Pluripotenzmarkern auf mRNA-Ebene im Vergleich zur alleinigen Cisplatin-Behandlung wider.
Somit präsentierte sich Tanespimycin als potenter Inhibitor von Krebsstammzellen. Auch wenn weitere vorklinische und klinische Tests und Untersuchungen erfolgen müssen, stellt Tanespimycin bzw. die Substanzklasse der HSP90-Inhibitoren einen interessanten und vielversprechenden Kandidaten für eine Ergänzung der Standardtherapie vor allem bei behandlungsresistenten und rekurrenten High-Risk-Hepatoblastomen dar.
Inflammation is a regulated reaction of the body to control a threat such as infection or injury. An efficient resolution of inflammation is critical to prevent the development of chronic inflammation and to restore tissue homeostasis. Macrophages (Mf) play a crucial role in the onset, but also in the resolution of inflammation, because they phagocytose and eliminate pathogens and tissue debris. Efficient efferocytosis, i.e. the engulfment of apoptotic cells, represents an important trigger for the onset of the resolution response and contributes to the pro-resolving reprogramming of Mf. Despite the importance of post- transcriptional modes of regulation during the resolution phase and translational control as a key node modulating gene expression in immune cells, relevant translational alterations remain largely elusive.
In the present study, I aimed to identify translationally regulated targets in inflammatory primary murine Mf upon resolution-promoting efferocytosis. To this end, I used total RNA-sequencing as well as de novo proteomics analyses to determine global transcriptional and translational changes. Sequencing data confirmed that efferocytosis induced a pro-resolution signature in inflammatory Mf and pointed towards translational regulation because the related integrated stress response was enriched upon efferocytosis. While changes of gene expression between efferocytic and non-efferocytic Mf appeared rather small at the transcriptional level, I observed considerable differences at the level of de novo synthesized proteins. This finding suggests a regulation at the level of translation. Furthermore, the tight connection between translational and metabolic changes was confirmed by enriched metabolism-associated terms of targets upregulated by efferocytosis at both RNA and de novo protein level. Interestingly, analysis of translationally regulated targets in response to inflammatory stimulation showed reduced translation for most targets, with only little impact of efferocytosis. Among those targets, I identified pro-resolving matrix metallopeptidase 12 (Mmp12) as a novel candidate, which showed translational repression during early inflammation and translational increase during the resolution phase. Noteworthy, a first indicator for a potential translation regulatory component of Mmp12 were the extremely high mRNA levels and not overly high de novo protein levels. Validation experiments recapitulated a slight elevation of Mmp12 mRNA expression and a significant downregulation of MMP12 intracellular protein levels in inflammatory Mf, as observed in the RNA-seq and de novo proteomics datasets. To investigate whether the discrepancy in mRNA and protein expression were due to changes in translation, I applied polysomal fractionation analysis to determine the translational status of Mmp12. Inflammatory Mf displayed a significantly lower relative Mmp12 mRNA abundance in the late polysomes compared to naïve Mf, suggesting reduced translational efficiency upon inflammatory stimulation. Consequently, extracellular MMP12 levels in the supernatant of inflammatory Mf decreased, although with a slight delay.
The functional impact of attenuated Mmp12 translation upon inflammatory stimulation was assessed in migration assays. While siRNA-mediated knockdown of Mmp12 did not alter Mf migration on uncoated plates, it increased migration 3-fold on matrigel/elastin-coated plates. Importantly, the increase in migrated distance driven by siMmp12 could be lowered by the addition of exogenous recombinant MMP12 protein. In line with reduced Mmp12 translation and MMP12 protein in inflammatory Mf, I observed a significant increase in cell migration on matrigel/elastin-coated plates, while it remained unaltered on uncoated plates. Consequently, Mf elastase MMP12 degrades elastin, thereby cell migration along elastin fibers is diminished. In inflammatory Mf, Mmp12 is translationally downregulated, thereby enhancing the migratory capacity.
In summary, the present study identifies a substantial contribution of translational regulation in the course of inflammation shown by high changes between inflammatory naïve and efferocytic Mf at the de novo proteomic level. Specifically, I was able to determine the translational regulation of pro-resolving Mmp12, which is repressed during early inflammation and recovers during the resolution phase. Functionally, translational control of MMP12 emerged as a strategy to alter the migratory properties of Mf, enabling enhanced, matrix- dependent migration of Mf during the early inflammatory phase, while restricting migration during the resolution phase.
Aryl hydrocarbon receptor-dependent and -independent pathways mediate curcumin anti-aging effects

The aryl hydrocarbon receptor (AhR) is a ligand-activated transcription factor whose activity can be modulated by polyphenols, such as curcumin. AhR and curcumin have evolutionarily conserved effects on aging. Here, we investigated whether and how the AhR mediates the anti-aging effects of curcumin across species. Using a combination of in vivo, in vitro, and in silico analyses, we demonstrated that curcumin has AhR-dependent or -independent effects in a context-specific manner. We found that in Caenorhabditis elegans, AhR mediates curcumin-induced lifespan extension, most likely through a ligand-independent inhibitory mechanism related to its antioxidant activity. Curcumin also showed AhR-independent anti-aging activities, such as protection against aggregation-prone proteins and oxidative stress in C. elegans and promotion of the migratory capacity of human primary endothelial cells. These AhR-independent effects are largely mediated by the Nrf2/SKN-1 pathway.
The pathophysiology of Takotsubo Syndrome (TTS) is not completely understood and the trigger of sudden cardiac death (SCD) in TTS is not clear either. We therefore sought to find an association between TTS and primary electrical diseases. A total of 148 TTS patients were analyzed between 2003 and 2017 in a bi-centric manner. Additionally, a literature review was performed. The patients were included in an ongoing retrospective cohort database. The coexistence of TTS and primary electrical diseases was confirmed in five cases as the following: catecholaminergic polymorphic ventricular tachycardia (CPVT, 18-year-old female) (n = 1), LQTS 1 (72-year-old female and 65-year-old female) (n = 2), LQTS 2 (17-year-old female) (n = 1), and LQTS in the absence of mutations (22-year-old female). Four patients suffered from malignant tachyarrhythmia and recurrent syncope after TTS. Except for the CPVT patient and one LQTS 1 patient, all other cases underwent subcutaneous ICD implantation. An event recorder of the CPVT patient after starting beta-blocker did not detect arrhythmias. The diagnosis of primary electrical disease was in 80% of cases unmasked on a TTS event. This diagnosis triggered a family clinical and genetic screening confirming the diagnosis of primary electrical disease. A subsequent literature review identified five cases as the following: a congenital atrioventricular block (n = 1), a Jervell and Lange-Nielsen Syndrome (n = 1), and a family LQTS in the absence of a mutation (n = 2), LQTS 2 (n = 1). A primary electrical disease should be suspected in young and old TTS patients with a family history of sudden cardiac death. In suspected cases, e.g., ongoing QT interval prolongation, despite recovery of left ventricular ejection fraction a family screening is recommended.
Test-Retest-Reliabilität der Präpulsinhibition (PPI) und PPI-Korrelation mit dem Arbeitsgedächtnis

Sensomotorisches Gating – ein Mechanismus zur Filterung des sensorischen Inputs und zur Regulierung des motorischen Outputs – wird experimentell durch die Präpulsinhibition (PPI) der akustisch ausgelösten Schreckreaktion (ASR) operatio-nalisiert. Frühere Studien deuten auf eine hohe Test-Retest-Reliabilität der PPI und eine mögliche Korrelation mit dem Arbeitsgedächtnis (engl. Working Memory (WM)) hin. Ziel dieser Studie war es, die Test-Retest-Reliabilität der PPI bei ge-sunden Menschen und ihre Korrelation mit der Leistung des WM zu überprüfen. Hier wurde ein akustisches Schreckreiz-PPI-Paradigma mit vier verschiedenen Präpuls-Intensitäten (64, 68, 72 und 76 dB(A)) und zwei verschiedene WM-Aufgaben (n-back, Change-Detection-Task (CDT)) verwendet. Es konnte eine ho-he Retest-Reliabilität der PPI mit einer mittleren Intraklassenkorrelation (engl. In-traclass Correlation (ICC)) von >.80 und eine signifikante positive Korrelation der PPI mit der n-back-, aber nicht mit der CDT-Leistung bestätigt werden. Eine detail-lierte Analyse zeigte, dass die PPI über alle Präpulsintensitäten hinweg sowohl mit den 2-back- als auch mit den 0-back-Bedingungen signifikant korrelierte, was auf eine Regulation durch konditionsübergreifende Prozesse (z. B. Aufmerksamkeit) schließen lässt. Wird jedoch die 0-back-Komponente aus den 2-back-Daten aus-partialisiert, sind spezifische und signifikante Korrelationen mit der Arbeitsgedächt-nisleistung für die 76 dB(A) PPI-Bedingung zu finden. Mit der vorliegenden Studie konnte die hohe Test-Retest-Reliabilität der PPI beim Menschen bestätigt und die Korrelation mit der Arbeitsgedächtnisleistung validiert und erweitert werden.
Whereas the lack of biomarkers in penile cancer (PeCa) impedes the development of efficacious treatment protocols, preliminary evidence suggests that c-MET and associated signaling elements may be dysregulated in this disorder. In the following study, we investigated whether c-MET and associated key molecular elements may have prognostic and therapeutic utility in PeCa. Formalin-fixed, paraffin-embedded tumor tissue from therapy-naïve patients with invasive PeCa was used for tissue microarray (TMA) analysis. Immunohistochemical staining was performed to determine the expression of the proteins c-MET, PPARg, β-catenin, snail, survivin, and n-MYC. In total, 94 PeCa patients with available tumor tissue were included. The median age was 64.9 years. High-grade tumors were present in 23.4%, and high-risk HPV was detected in 25.5%. The median follow-up was 32.5 months. High expression of snail was associated with HPV-positive tumors. Expression of β-catenin was inversely associated with grading. In both univariate COX regression analysis and the log-rank test, an increased expression of PPARg and c-MET was predictive of inferior disease-specific survival (DSS). Moreover, in multivariate analysis, a higher expression of c-MET was independently associated with worse DSS. Blocking c-MET with cabozantinib and tivantinib induced a significant decrease in viability in the primary PeCa cell line UKF-PeC3 isolated from the tumor tissue as well as in cisplatin- and osimertinib-resistant sublines. Strikingly, a higher sensitivity to tivantinib could be detected in the latter, pointing to the promising option of utilizing this agent in the second-line treatment setting.
The cellular composition of the tumor microenvironment, including tumor, immune, stromal, and endothelial cells, significantly influences responses to cancer therapies. In this study, we analyzed the impact of oxidative stress, induced by cold atmospheric plasma (CAP), on tumor cells, T cells, and macrophages, which comprise part of the melanoma microenvironment. To accomplish this, cells were grown in different in vitro cell culture models and were treated with varying amounts of CAP. Subsequent alterations in viability, proliferation, and phenotype were analyzed via flow cytometry and metabolic alterations by Seahorse Cell Mito Stress Tests. It was found that cells generally exhibited reduced viability and proliferation, stemming from CAP induced G2/M cell cycle arrest and subsequent apoptosis, as well as increased mitochondrial stress following CAP treatment. Overall, sensitivity to CAP treatment was found to be cell type dependent with T cells being the most affected. Interestingly, CAP influenced the polarization of M0 macrophages to a “M0/M2-like” phenotype, and M1 macrophages were found to display a heightened sensitivity to CAP induced mitochondrial stress. CAP also inhibited the growth and killed melanoma cells in 2D and 3D in vitro cell culture models in a dose-dependent manner. Improving our understanding of oxidative stress, mechanisms to manipulate it, and its implications for the tumor microenvironment may help in the discovery of new therapeutic targets.
In this review article, we will first provide a brief overview of the ErbB receptor–ligand system and its importance in developmental and physiological processes. We will then review the literature regarding the role of ErbB receptors and their ligands in the maladaptive remodeling of lung tissue, with special emphasis on idiopathic pulmonary fibrosis (IPF). Here we will focus on the pathways and cellular processes contributing to epithelial–mesenchymal miscommunication seen in this pathology. We will also provide an overview of the in vivo studies addressing the efficacy of different ErbB signaling inhibitors in experimental models of lung injury and highlight how such studies may contribute to our understanding of ErbB biology in the lung. Finally, we will discuss what we learned from clinical applications of the ErbB1 signaling inhibitors in cancer in order to advance clinical trials in IPF.
Children’s and adolescents’ lives drastically changed during COVID lockdowns worldwide. To compare accident- and injury-related admissions to pediatric intensive care units (PICU) during the first German COVID lockdown with previous years, we conducted a retrospective multicenter study among 37 PICUs (21.5% of German PICU capacities). A total of 1444 admissions after accidents or injuries during the first lockdown period and matched periods of 2017–2019 were reported and standardized morbidity ratios (SMR) were calculated. Total PICU admissions due to accidents/injuries declined from an average of 366 to 346 (SMR 0.95 (CI 0.85–1.05)). Admissions with trauma increased from 196 to 212 (1.07 (0.93–1.23). Traffic accidents and school/kindergarten accidents decreased (0.77 (0.57–1.02 and 0.26 (0.05–0.75)), whereas household and leisure accidents increased (1.33 (1.06–1.66) and 1.34 (1.06–1.67)). Less neurosurgeries and more visceral surgeries were performed (0.69 (0.38–1.16) and 2.09 (1.19–3.39)). Non-accidental non-suicidal injuries declined (0.73 (0.42–1.17)). Suicide attempts increased in adolescent boys (1.38 (0.51–3.02)), but decreased in adolescent girls (0.56 (0.32–0.79)). In summary, changed trauma mechanisms entailed different surgeries compared to previous years. We found no evidence for an increase in child abuse cases requiring intensive care. The increase in suicide attempts among boys demands investigation.
Mucormycosis is an invasive fungal infection associated with high mortality, partly due to delayed diagnosis and inadequate empiric therapy. As fungal cultures often fail to grow Mucorales, identification of respective hyphae in tissue is frequently needed for diagnosis but may be challenging. We studied fluorescence in situ hybridization (FISH) targeting specific regions of the fungal ribosomal RNA (rRNA) of Mucorales to improve diagnosis of mucormycosis from tissue samples. We generated a probe combination specifically targeting Mucorales. Probe specificity was verified in silico and using cultivated fungi. Mucorales hyphae in tissue of a mouse model demonstrated a bright cytoplasmatic hybridization signal. In tissue samples of patients with mucormycosis, a positive signal was seen in 7 of 12 (58.3%) samples. However, autofluorescence in 3 of 7 (42.9%) samples impaired the diagnostic yield. Subsequent experiments suggested that availability of nutrients and antifungal therapy may impact on the FISH signal obtained with Mucorales hyphae. Diagnosis of mucormycosis from tissue might be improved by rRNA FISH in a limited number of cases only. FISH signals may reflect different wphysiological states of fungi in tissue. Further studies are needed to define the value of FISH to diagnose mucormycosis from other clinical samples.
Das Harnblasenkarzinom ist einer der häufigsten Tumoren weltweit. Insbesondere die muskelinvasiven Tumoren haben eine schlechte Prognose und stellen sich morphologisch sehr unterschiedlich dar. Diese Heterogenität wird bislang bei Therapieentscheidungen nicht beachtet. Um Patienten zukünftig individuell auf den vorliegenden Subtyp des muskelinvasiven Harnblasenkarzinoms (MIBC, muscle invasive bladder cancer) behandeln zu können und dadurch unnötige Belastungen durch Chemotherapien vermeiden zu können, ist eine einfache und kostengünstige Diagnostik erforderlich. Das Ziel der vorliegenden Arbeit war es, den Einsatz bestimmter immunhistochemischer Färbungen als ein mögliches diagnostisches Routineverfahren zur Bestimmung der vorliegenden Subtypen auszutesten. Hierzu wurde die Expression von „luminalen“ und „basalen“ Proteinen mit histologischen Subtypen des MIBCs korreliert. In einem zweiten Schritt wurde der Einfluss auf das Überleben mit und ohne adjuvante Chemotherapie untersucht.
Es wurden insgesamt 181 Tissue-Microarray-Spots analysiert. Alle histologischen Patientenproben sowie klinischen Daten stammten aus dem Universitätsklinikum Frankfurt am Main. Aus den entsprechenden Gewebeblöcken wurden Stanzen zur Erstellung eines Tissue-Microarrays (TMAs) entnommen. Diese wurden konventionell angeschnitten und histologisch mit Hämatoxylin-Eosin (H/E) sowie immunhistochemisch mit Cytokeratin 5/6 (CK5/6), Cytokeratin 20 (CK20), Glutamyl Aminotransferase-Untereinheit A bindendem Protein 3 (GATA3), Tumorsuppressor- protein p53 und Synaptophysin (SYNAPT) gefärbt.
Anhand der H/E-Schnitte wurden die vorliegenden histologischen Subtypen lichtmikroskopisch bestimmt und es folgte eine statistische Auswertung der Färbeergebnisse. Die deskriptive statistische Analyse zeigte insbesondere für die beiden Färbungen CK5/6 und GATA3 signifikante Ergebnisse. Deshalb wurden in einem zweiten Schritt alle Fälle der Studienkohorte den bekannten vier Gruppen: CK5/6 positiv, GATA3 positiv, doppelt positiv und doppelt negativ zugeordnet und näher untersucht. Es folgte eine Überlebensanalyse nach Kaplan Meier Schätzer sowie uni- und multivariate Analysen.
Die Ergebnisse zeigten eine Assoziation der Expression von CK5/6 mit einer squamösen Differenzierung (96%) und eine Assoziation der Expression von GATA3 mit einer mikropapillären Differenzierung (100%). Die adjuvante Chemotherapie ging mit einem Überlebensvorteil (HR 0,15 95%KI 0,1-0,3; p<0,001) der Patienten mit MIBC einher. Immunhistochemisch doppelt negative Patienten mit MIBC wiesen ein verringertes Gesamtüberleben auf (HR 4,96; 95%KI 1,6-15,6; p=0,006); in der Gruppe der doppelt negativen MIBCs fanden sich fünf verschiedene histologische Subtypen. Zusammenfassend lässt sich sagen, dass die immunhistochemische Klassifizierung des MIBCs mit histologischen Subtypen assoziiert ist und dabei helfen kann, Fälle in der pathologischen Routine in „luminal“ und „basal“ zu unterteilen. Jedoch ist ein auf zwei Markern basierendes Klassifizierungssystem nicht ausreichend, um die Heterogenität des MIBCs abzubilden und die Basis für Therapieentscheidungen zu bilden.
Background: High reproducibility and low intra- and interobserver variability are important strengths of cardiac magnetic resonance (CMR). In clinical practice a significant learning curve may however be observed. Basic CMR courses offer an average of 1.4 h dedicated to lecturing and demonstrating left ventricular (LV) function analysis. The purpose of this study was to evaluate the effect of initial teaching on complete and intermediate beginners’ quantitative measurements of LV volumes and function by CMR.
Methods: Standard clinical cine CMR sequences were acquired in 15 patients. Five observers (two complete beginners, one intermediate, two experienced) measured LV volumes. Before initial evaluation beginners read the SCMR guidelines on CMR analysis. After initial evaluation, beginners participated in a two-hour teaching session including cases and hands-on training, representative for most basic CMR courses, after which it is uncertain to what extent different centres provide continued teaching and feedback in-house. Dice Similarity Coefficient (DSC) assessed delineations. Agreement, accuracy, precision, repeatability and reliability were assessed by Bland-Altman, coefficient of variation, and intraclass correlation coefficient methods.
Results: Endocardial DSC improved after teaching (+0.14 ± 0.17;p < 0.001) for complete beginners. Low intraobserver variability was found before and after teaching, however with wide limits of agreement. Beginners underestimated volumes by up to 44 ml (EDV), 27 ml (ESV) and overestimated LVM by up to 53 g before teaching, improving to an underestimation of up to 9 ml (EDV), 7 ml (ESV) and an overestimation of up to 30 g (LVM) after teaching. For the intermediate beginner, however, accuracy was quite high already before teaching.
Conclusions: Initial teaching to complete beginners increases accuracy for assessment of LV volumes, however with high bias and low precision even after standardised teaching as offered in most basic CMR courses. Even though the intermediate beginner showed quite high accuracy already before teaching, precision did generally not improve after standardised teaching. To maintain CMR as a technique known for high accuracy and reproducibility and low intra- and inter-observer variability for quantitative measurements, internationally standardised training should be encouraged including high-quality feedback mechanisms. Objective measurements of training methods, training duration and, above all, quality of assessments are required.
Low-caloric formula diets can improve hemodynamic parameters of patients with type 2 diabetes. We, therefore, hypothesized that persons with overweight or obesity can benefit from a high-protein, low-glycemic but moderate-caloric formula diet. This post-hoc analysis of the Almased Concept against Overweight and Obesity and Related Health Risk- (ACOORH) trial investigated the impact of a lifestyle intervention combined with a formula diet (INT, n = 308) compared to a control group with lifestyle intervention alone (CON, n = 155) on hemodynamic parameters (systolic and diastolic blood pressure (SBP, DBP), resting heart rate (HR), and pulse wave velocity (PWV)) in high-risk individuals with prehypertension or hypertension. INT replaced meals during the first 6 months (1 week: 3 meals/day; 2–4 weeks: 2 meals/day; 5–26 weeks: 1 meal/day). Study duration was 12 months. From the starting cohort, 304 (68.3%, INT: n = 216; CON: n = 101) participants had a complete dataset. Compared to CON, INT significantly reduced more SBP (−7.3 mmHg 95% CI [−9.2; −5.3] vs. −3.3 mmHg [−5.9; −0.8], p < 0.049) and DBP (−3.7 mmHg [−4.9; −2.5] vs. −1.4 mmHg [−3.1; 0.2], p < 0.028) after 12 months. Compared to CON, INT showed a pronounced reduction in resting HR and PWV after 6 months but both lost significance after 12 months. Changes in SBP, DBP, and PWV were significantly associated positively with changes in body weight and fat mass (all p < 0.05) and resting HR correlated positively with fasting insulin (p < 0.001) after 12 months. Combining a lifestyle intervention with a high-protein and low-glycemic formula diet improves hemodynamic parameters to a greater extent than lifestyle intervention alone in high-risk individuals with overweight and obesity.
Soil-Transmitted Helminths (STH) is a group of nematodes that infect people and transmitted through soil media. STH occurs especially among pre-school and school-aged children, and commonly related to environmental sanitation and personal hygiene. The study objected to determine the factors related to the incidence of STH in children 5-15 years who lived surrounding the Sukawinatan district of Palembang city. The observational analytic using the cross-sectional design, consisted of 110 subjects sampled by consecutive sampling. Data on environmental sanitation and personal hygiene were obtained by questionnaires, while infection status using the Kato-Katz faecal technic. The results were analyzed using Chi-square test (α = 0.05), showed that 24.5% of population where infected with STH. A number of 1-24-2 children were infected with hookworm-Ascaris lumbricoides-Trichuris trichiura infection, respectively. Based on statistical test results, the association of STH infection with variables were: waste disposal (p = 0.268), water facilities (p = 1.000), sewage disposal (p = 0.224), latrine (p = 0.021), hand washing prior to meal (p = 0.001), hand washing after defecate (p = 0.028), use of footwear (p = 0.013), and nail hygiene (p = 1.000). Concluded that the significant factors related to STH were use of latrine, hand washing behaviour, and use of footwear. Further research will be necessary to successfully eliminate this neglected tropical disease.
Introduction: Older patients with multimorbidity, polypharmacy and related complex care needs represent a growing proportion of the population and a challenge for healthcare systems. Particularly in transitional care (hospital admission and hospital discharge), medical errors, inappropriate treatment, patient concerns and lack of confidence in healthcare are major problems that may arise from a lack of information continuity. The aim of this study is to develop an intervention to improve informational continuity of care at the interface between general practice and hospital care.
Methods and analysis: A qualitative approach will be used to develop our participatory intervention. Overall, 32 semistructured interviews with relevant stakeholders will be conducted and analysed. The stakeholders will include healthcare professionals from the outpatient setting (general practitioners, healthcare assistants, ambulatory care nurses) and the inpatient setting (clinical doctors, nurses, pharmacists, clinical information scientists) as well as patients and informal caregivers. At a series of workshops based on the results of the stakeholder analyses, we aim to develop a participatory intervention that will then be implemented in a subsequent pilot study. The same stakeholder groups will be invited for participation in the workshops.
Ethics and dissemination: Ethical approval for this study was waived by the Ethics Committee of Goethe University Frankfurt because of the nature of the proposed study. Written informed consent will be obtained from all study participants prior to participation. Results will be tested in a pilot study and disseminated at (inter)national conferences and via publication in peer-reviewed journals.
Die vorliegende Arbeit behandelt den Vergleich zweier Geräte - „Endotrust MiFusion TLS 2“ und „Medtronic LigaSure Maryland System“ - zur endoskopischen Entnahme der Arteria radialis (RA) zur Verwendung als Bypass-Gefäß in der Herzchirurgie.
Grundsätzlich kommen in der Bypass-Chirurgie zur Herstellung eines Free-Grafts am Herzen neben der Verwendung der Thoraxarterien die Vena Saphena Magna (VSM) sowie die RA in Frage. In den aktuellen europäischen Leitlinien zur Behandlung von hochgradigen Stenosen wird die Verwendung der RA empfohlen („Class 1 Level B“-Empfehlung).
Die Frage, ob die RA für den Einsatz als Bypass-Gefäß offen oder endoskopisch entnommen werden sollte, ist in der Literatur weiterhin umstritten. In den aktuellen Leitlinien zur Behandlung von koronaren Herzkrankheiten wird aufgrund dieser insoweit uneindeutigen Studienlage keine Empfehlung ausgesprochen. Trotzdem ist die endoskopische Entnahme der RA im klinischen Alltag mittlerweile etabliert. Im Kontext dieser uneindeutigen Studienlage einerseits und der praktischen Bedeutung endoskopischer Entnahme andererseits ist es Zielsetzung der vorliegenden Arbeit, durch einen Gerätevergleich einen Beitrag zur Optimierung der endoskopischen Operationstechnik zu leisten. Es soll zudem aufgezeigt werden, inwieweit die endoskopische Entnahme der RA ein sicheres und effizientes Verfahren darstellt.
In der Literatur zum Vergleich von Operationstechniken zur Entnahme von Bypass-Gefäßen werden häufig histologische Untersuchungen angewendet. Diese ermöglichen eine zeitnahe Beurteilung der Qualität des entnommenen Grafts. Das ist auch in dieser Arbeit der wesentliche Grund dafür, dass die histologische Beurteilung der Qualität der RA als primärer Endpunkt angewendet wird. In Anlehnung an die Literatur wurde die strukturelle Integrität des Endothels und der Elastica interna beurteilt. Die histologische Beurteilung erfolgte nach Immunfluoreszenz-Bearbeitung der Proben.
Im Einklang mit der bestehenden Literatur zur Frage, ob die RA offen oder endoskopisch entnommen werden sollte, wurde in dieser Studie als sekundäre Endpunkte Kriterien bezüglich der Sicherheit und Effizienz der Entnahme verwendet. Dies betrifft das Auftreten von intra- und postoperativen Komplikationen, insbesondere im Hinblick auf neurologische Beeinträchtigungen am operierten Arm, sowie die Entnahmedauer und den operativen Aufwand zur Blutstillung.
Die in dieser Arbeit behandelte Studie wurde als prospektive, 1:1 randomisierte Studie mit zwei Gruppen mit jeweils 50 Patienten durchgeführt. Alle Operationen erfolgten im Zeitraum Januar 2017 bis Juli 2017 im Herzzentrum der Kerckhoff-Klinik Bad Nauheim.
Bezüglich der Resultate des Gerätevergleichs zeigte sich ein eindeutiges Ergebnis. Sämtliche Beurteilungskriterien, bei denen signifikante Unterschiede zwischen den beiden Gruppen aufgetreten sind, u.a. Integrität der Elastica interna, Entnahmedauer, Vorkommen von Residualblutungen, Auftreten von sensorischen Störungen fallen zugunsten des LigaSure-Systems aus.
Dabei ist unserer Meinung nach der wichtigste Einzelaspekt, dass die histologisch ermittelte Integrität der Elastica interna als Indikator für die Qualität des entnommen Grafts in der LigaSure-Gruppe signifikant besser war als in der MiFusion-Gruppe. Dagegen konnten aus der histologischen Untersuchung des Endothels keine klaren Rückschlüsse gezogen werden. Insofern besteht Unsicherheit, ob die Schädigung der Endothelschicht durch die Entnahme selbst oder durch die anschließende Präparierung der entnommenen Proben verursacht wurde.
Bezüglich der sekundären Endpunkte zeigten sich für die Mifusion-Patientengruppe im Vergleich zu anderen Studien zur endoskopischen Entnahme der RA zufriedenstellende bis gute und für die LigaSure Gruppe gute bis sehr gute Ergebnisse. Unabhängig vom verwendeten Gerät kann diese Studie deshalb als eine Bestätigung bisheriger Studien zur Vorteilhaftigkeit der endoskopischen RA-Entnahme angesehen werden. Letztlich fehlt jedoch weiterhin der Nachweis, dass eine endoskopische Entnahme unbedenklich im Hinblick auf den langfristigen kardiologischen Outcome ist. Dies bleibt zukünftiger Forschung vorbehalten.
Darüber hinaus trägt die Studie dazu bei, das klinische Erfahrungswissen über operationstechnische Details bei der Entnahme der Radialarterie zu erweitern und somit die Akzeptanz für die Verwendung der Radialarterie als Bypass-Gefäß in der koronaren Herzchirurgie zu verbessern.
Background: Particulate matter (PM) emission caused by tobacco combustion leads to severe health burdens worldwide. Second-hand smoke exposure is extraordinarily high in enclosed spaces (e.g., indoor rooms, car cabins) and poses a particular threat to the health of vulnerable individuals (e.g., children, elderly, etc.). This study aimed to establish a new measuring platform and investigate PM emissions under four different ventilation conditions inside a car cabin without exposing any person to harmful tobacco smoke.
Methods: PM concentrations were measured during the smoking of 3R4F reference cigarettes in a Mitsubishi Space Runner (interior volume 3.709 m3). The cigarettes were smoked with a machine, eliminating exposure of the researchers. Cigarettes were extinguished 4.5 min after ignition, and PM measurements continued until 10 min after ignition.
Results: High mean PM concentrations were measured for cigarettes without ventilation after 4.5 min (PM10: 1150 µg/m3, PM2.5: 1132 µg/m3, PM1: 861.6 µg/m3) and after 10 min (PM10: 1608 µg/m3, PM2.5: 1583 µg/m3, PM1: 1133 µg/m3). 3R4F smoked under conditions with turned on ventilation resulted in reduction of PM compared to those smoked without ventilation after 4.5 min (PM10:-47.5 to -58.4%, PM2.5:-47.2 to -58%, PM1:-39.6 to -50.2%) and after 10 min (PM10:-70.8 to -74.4%, PM2.5:-70.6 to -74.3%, PM1:-64.0 to -68.0%). Cigarettes smoked without ventilation generated high PM peaks at 4.5 min (PM10: 2207 µg/m3, PM2.5: 2166 µg/m3, PM1: 1421 µg/m3) and at 10 min (PM10: 1989 µg/m3, PM2.5: 1959 µg/m3, PM1: 1375 µg/m3). PM peaks of cigarettes smoked under different ventilation modes varied at 4.5 min (PM10: 630-845 µg/m3, PM2.5: 625-836 µg/m3, PM1: 543 - 693 µg/m3) and 10 min (PM10: 124 - 130 µg/m3, PM2.5: 124 - 129 µg/m3, PM1: 118 - 124 µg/m3).
Conclusion: The new measuring platform provides a safer way for researchers to investigate PM emissions of cigarettes. These data are comparable to published research and show that smoking in a parked vehicle with the windows closed generates harmful PM emissions even when the vehicle ventilation is in operation. Future studies should be carried out using the new measuring platform investigating PM exposure and PM distribution of in-vehicle smoking under a wide range of conditions.
Background: A good physician should be empathic and altruistic, among other qualities. Therefore, the levels of socially undesirable personality traits (Dark Triad) as well as implicit motives of achievement, affiliation and power (Multi-Motive Grid) among medical students as future physicians were analyzed at two different points in their medical training.
Methods: This study includes 380 medical students in their first year and 217 in their third year in Germany. All participants completed the Dirty Dozen (DD) and Multi-Motive Grid (MMG) questionnaires at the end of two different classes as paper-and-pencil tests. Relevant differences of the Dark Triad traits between the medical students and reference sample and the two different cohorts, as well as their implicit motives, the associations of Dark Triad traits and MMG components and gender differences of the Dark Triad traits were calculated.
Results: There were no significant group differences between year one and year three medical students in narcissism, psychopathy and Machiavellianism (Dark Triad). There were no significant differences between the medical students and reference sample except in psychopathy. Male students scored significantly higher in the Dark Triad traits than female students. In the MMG, first-year students scored significantly higher levels in Fear of Rejection, and lower levels in Hope of Success and Hope of Power than the third-year students. Some associations were found between narcissism and Machiavelliansim with Hope of Success, Hope of Power and Fear of power.
Conclusions: Dark Triad traits already appear to exist before the commencement of medical studies. These traits do not differ significantly between the medical students and reference sample; only a few MMG components seem to differ at different stages of their studies. This lack of differences between the medical students and validation cohort indicates that tests based on (undesirable) personality traits are not suitable criteria for the admission selection of medical students.
Tobacco smoke-associated particulate matter emissions in a car cabin using the TAPaC platform

Zigarettenrauch enthält bis zu 5000 Inhaltsstoffe, von denen mindestens 250 gesundheits-schädlich und 98 krebserregend sind. Der freigesetzte Feinstaub erreicht hohe Konzentrati-onen in Innenräumen und ist somit besonders schädlich für Passivraucher (z.B. Kinder).
Ziel war es eine Messplattform zu etablieren, mit dessen Hilfe die Feinstaubexposition durch Zigarettenrauch im Fahrzeuginnenraum unter unterschiedlichen ventilatorischen Szenarien untersucht werden konnte. Zudem sollte der Einfluss verschiedener Tabakprodukte auf die Feinstaubkonzentration getestet werden.
Im ersten Teil dieser Dissertation wird die neuartige TAPaC Messplattform (tobacco-associated particulate matter emissions inside a car cabin: establishment of a new measuring platform) vorgestellt. Sie erlaubt die Auswirkungen verschiedener ventilatorischer Sze-narien auf die Feinstaubemission von Zigarettenrauch im Auto besser beurteilen zu können. Da niemand gesundheitsschädlichem Tabakrauch ausgesetzt wird, kann sie ohne jegliche ethische Bedenken eingesetzt werden. Die Zigaretten werden hierbei einzeln auf der Beifahrerseite verraucht. Der beim Rauchen freigesetzte Feinstaub wird auf der Fahrerseite gemessen und in PM10 (Partikel mit einem aerodynamischen Durchmesser <10 µm), PM2,5 (Partikel mit einem aerodynamischen Durchmesser <2,5 µm), und PM1 (Partikel mit einem aerodynamischen Durchmesser <1 µm) unterteilt. Hierbei konnten unter der Verwendung von 3R4F Research Cigarettes extrem hohe Feinstaubmesswerte bei geschlossenen Fenstern und ausgeschalteter Lüftung nachgewiesen werden (PM10: 1608 µg/m3, PM2,5: 1583 µg/m3, PM1: 1133 µg/m3). Diese Daten stellen Durchschnittwerte nach 10-minütiger Messung dar. Auch konnte eine Reduktion der Feinstaubkonzentration (PM10: -70,8 bis -74,4%, PM2,5: -70,6 bis -74,3%, PM1: -64,0 bis -68,0%) durch den Einsatz der Lüftung nachgewiesen werden.
Der zweite Teil dieser Dissertation befasste sich maßgeblich mit dem Einfluss unterschiedlicher ventilatorischer Szenarien auf die Feinstaubkonzentrationen im Auto. Unter Verwen-dung von drei unterschiedlichen Zigarettenprodukten (3R4F Research Cigarettes, Marlboro Red, Marlboro Gold) wurden insgesamt 7 ventilatorische Szenarien (Condition C1b–C7b) getestet. Für alle Szenarien, mit Ausnahme von C1b, war die Autolüftung auf Stufe 2/4 ge-stellt und in Richtung der Windschutzscheibe gerichtet. Die Szenarien beinhalteten: Condition 1 (C1b) Fenster geschlossen, Autolüftung aus und externer Ventilator aus, Condition 2 (C2b) Fenster 10 cm geöffnet und externer Ventilator aus, Condition 3 (C3b) Fenster 10 cm geöffnet und externer Ventilator auf höchster Stufe (3/3) an, Condition 4 (C4b) Fenster halb geöffnet und externer Ventilator aus, Condition 5 (C5b) Fenster halb geöffnet und externer Ventilator auf höchster Stufe (3/3) an, Condition 6 (C6b) Fenster vollständig geöffnet und externer Ventilator aus und Condition 7 (C7b) Fenster vollständig geöffnet und externer Ventilator auf höchster Stufe (3/3) an.
Es zeigten sich besonders hohe Feinstaubkonzentrationen bei Zigaretten, welche ohne Ventilation bei geschlossenem Fenster verraucht wurden. Unabhängig von der verwendeten Marke war die Feinstaubbelastung nach 10 min unter C1b (PM10: 1272–1697 µg/m3, PM2,5: 1253–1659 µg/m3, PM1: 964–1263 µg/m3) deutlich höher als unter C2b (PM10: 67–84 µg/m3, PM2,5: 68–83 µg/m3, PM1: 66–79 µg/m3), C3b (PM10: 100–139 µg/m3, PM2,5: 99–138 µg/m3, PM1: 95–132 µg/m3), C4b (PM10: 84–94 µg/m3, PM2,5: 84–93 µg/m3, PM1: 81–89 µg/m3), C5b (PM10: 94–120 µg/m3, PM2,5: 93–119 µg/m3, PM1: 90–114 µg/m3), C6b (PM10: 155–196 µg/m3, PM2,5: 154–195 µg/m3, PM1: 148–184 µg/m3), und C7b (PM10: 74–99 µg/m3, PM2,5: 72–97 µg/m3, PM1: 69–93 µg/m3). Ebenfalls wurden Feinstaubspitzenwerte bei 4,5 min und 10 min ausgewertet. Bei 4,5 min konnte PM10 unter C2b–C7b um 81,6–93,3% im Vergleich zu C1b reduziert werden. Bei 10 min sogar um 92,9–99,3%. Die 3R4F Zigarette hatte die höchste Feinstaubemission gefolgt von Marlboro Rod und Marlboro Gold. Zudem zeigte sich, dass die Feinstaubemission auch von den Tabakinhaltsstoffen und dessen Konzentrationen abhängig ist.
Die Etablierung der neuen Messplattform ermöglicht die Erforschung von Feinstaubexpositionen durch Zigarettenrauch im Auto ohne potentiell gesundheitliche Folgen. In Zusammenschau der Daten ließ sich nach Öffnen des Fensters und unter Einflussnahme verschiedener ventilatorischer Einstellungen eine deutliche Reduktion der Feinstaubkonzentration im Auto feststellen. Nichtsdestotrotz bleibt die Feinstaubbelastung im Autoinnenraum zu hoch und übersteigt die Richtwerte der Air Quality Guidelines aus dem Jahre 2021 der WHO. Die experimentell untersuchten Belüftungsszenarien sind somit insuffizient, da sie nicht vor der toxischen Feinstaubexposition durch Passivrauch beim Autofahren schützen können.
Background/aim: The aim of this study was to analyze a population of patients who had suffered from traumatic dental injuries (TDIs) by using different patient-, trauma- and treatment-related parameters.
Material and methods: All dental records of patients ≥ 3 years old who had presented at the dental emergency service between Jan 1, 2009 and Dec 31, 2016 for the treatment of dental trauma were analyzed. A total of 2758 patients were invited for a recall examination at the Department for Dental Surgery and Implantology, ZZMK Carolinum, Goethe University Frankfurt, Germany; of these, 269 patients attended their recall appointments.
Results: The enrolled patient population consisted of 1718 males and 1040 females, with a mean age of 19.63 years (median 12.00 ± 17.354 years). A total of 4909 injured teeth were assessed, with a mean of 1.78 injured teeth per patient (median 2.00 ± 1.279). Males were found to be more frequently affected by TDIs compared to females (1.65:1). The majority of these injuries occurred in the first two decades of life (66.1%; n = 1824). The majority of the patients presented for initial treatment within 24 h of their accident (95.7%). The most frequent TDIs were isolated luxation injuries 49.4% (n = 2426) and isolated crown fractures 30% (n = 1472). Combination injuries were diagnosed in 20.6% of the cases (n = 1011).
Conclusions: Based on the findings of the present analysis, it can be concluded that males were more frequently affected by TDIs than females. Most patients had suffered from TDI before they had turned 10 years of age. Overall, the enamel–dentin fracture was found to be the most frequent injury, followed by concussions and lateral luxations.
Oncogenic transformation of lung epithelial cells is a multi-step process, frequently starting with the inactivation of tumor suppressors and subsequent activating mutations in proto-oncogenes, such as members of the PI3K or MAPK family. Cells undergoing transformation have to adjust to changes, such as metabolic requirements. This is achieved, in part, by modulating the protein abundance of transcription factors, which manifest these adjustments. Here, we report that the deubiquitylase USP28 enables oncogenic reprogramming by regulating the protein abundance of proto-oncogenes, such as c-JUN, c-MYC, NOTCH and ΔNP63, at early stages of malignant transformation. USP28 is increased in cancer compared to normal cells due to a feed-forward loop, driven by increased amounts of oncogenic transcription factors, such as c-MYC and c-JUN. Irrespective of oncogenic driver, interference with USP28 abundance or activity suppresses growth and survival of transformed lung cells. Furthermore, inhibition of USP28 via a small molecule inhibitor reset the proteome of transformed cells towards a ‘pre-malignant’ state, and its inhibition cooperated with clinically established compounds used to target EGFRL858R, BRAFV600E or PI3KH1047R driven tumor cells. Targeting USP28 protein abundance already at an early stage via inhibition of its activity therefore is a feasible strategy for the treatment of early stage lung tumours and the observed synergism with current standard of care inhibitors holds the potential for improved targeting of established tumors.
Multiplex families with a high prevalence of a psychiatric disorder are often examined to identify rare genetic variants with large effect sizes. In the present study, we analysed whether the risk for bipolar disorder (BD) in BD multiplex families is influenced by common genetic variants. Furthermore, we investigated whether this risk is conferred mainly by BD-specific risk variants or by variants also associated with the susceptibility to schizophrenia or major depression. In total, 395 individuals from 33 Andalusian BD multiplex families (166 BD, 78 major depressive disorder, 151 unaffected) as well as 438 subjects from an independent, BD case/control cohort (161 unrelated BD, 277 unrelated controls) were analysed. Polygenic risk scores (PRS) for BD, schizophrenia (SCZ), and major depression were calculated and compared between the cohorts. Both the familial BD cases and unaffected family members had higher PRS for all three psychiatric disorders than the independent controls, with BD and SCZ being significant after correction for multiple testing, suggesting a high baseline risk for several psychiatric disorders in the families. Moreover, familial BD cases showed significantly higher BD PRS than unaffected family members and unrelated BD cases. A plausible hypothesis is that, in multiplex families with a general increase in risk for psychiatric disease, BD development is attributable to a high burden of common variants that confer a specific risk for BD. The present analyses demonstrated that common genetic risk variants for psychiatric disorders are likely to contribute to the high incidence of affective psychiatric disorders in the multiplex families. However, the PRS explained only part of the observed phenotypic variance, and rare variants might have also contributed to disease development.
DNA methylation profiles of aggressive behavior may capture lifetime cumulative effects of genetic, stochastic, and environmental influences associated with aggression. Here, we report the first large meta-analysis of epigenome-wide association studies (EWAS) of aggressive behavior (N = 15,324 participants). In peripheral blood samples of 14,434 participants from 18 cohorts with mean ages ranging from 7 to 68 years, 13 methylation sites were significantly associated with aggression (alpha = 1.2 × 10−7; Bonferroni correction). In cord blood samples of 2425 children from five cohorts with aggression assessed at mean ages ranging from 4 to 7 years, 83% of these sites showed the same direction of association with childhood aggression (r = 0.74, p = 0.006) but no epigenome-wide significant sites were found. Top-sites (48 at a false discovery rate of 5% in the peripheral blood meta-analysis or in a combined meta-analysis of peripheral blood and cord blood) have been associated with chemical exposures, smoking, cognition, metabolic traits, and genetic variation (mQTLs). Three genes whose expression levels were associated with top-sites were previously linked to schizophrenia and general risk tolerance. At six CpGs, DNA methylation variation in blood mirrors variation in the brain. On average 44% (range = 3–82%) of the aggression–methylation association was explained by current and former smoking and BMI. These findings point at loci that are sensitive to chemical exposures with potential implications for neuronal functions. We hope these results to be a starting point for studies leading to applications as peripheral biomarkers and to reveal causal relationships with aggression and related traits.

The car dependence of people living in contemporary cities is a major concern for policy makers, who often find it difficult to persuade people into more sustainable transport modes. By contrast, recent insights from neuroscience have shown that a broad spectrum of behaviors can become habitual and, thus, resistant to change. Here, we outline the potential of collaboration between neuroscience and human geography aiming at a better understanding of habits that determine everyday commuting routines.
Neurodevelopmental psychiatric disorders (NPDs) like attention deficit hyperactivity disorder (ADHD), autism spectrum disorder (ASD), and schizophrenia, affect millions of people worldwide. Despite recent progress in NPD research, much remains to be discovered about their underpinnings, therapeutic targets, effects of biological sex and age. Risk factors influencing brain development and signalling include prenatal inflammation and genetic variation. This dissertation aimed to build upon these findings by combining behavioural, molecular, and neuromorphological investigations in mouse models of such risk factors, i.e. maternal immune activation (MIA), neuron-specific overexpression (OE) of the cytoplasmatic isoforms of the RNA-binding protein RBFOX1, and neuronal deletion of the small Ras GTPase DIRAS2.
Maternal infections during pregnancy pose an increased risk for NPDs in the offspring. While viral-like MIA has been previously established elsewhere, this study was the first in our institution to implement the model. I validated NPD-relevant deficits in anxiety- and depression-like behaviours, as well as dose- and sex-specific social deficits in mouse offspring following MIA in early gestation. Proteomic analyses in embryonic and adult hippocampal (HPC) synaptoneurosomes highlighted novel and known targets affected by MIA. Analysis of the embryonic dataset implicated neurodevelopmental disruptions of the lipid, polysaccharide, and glycoprotein metabolism, important for proper membrane function, signalling, and myelination, for NPD-pertinent sequelae. In adulthood, the observed changes encompassed transmembrane trafficking and intracellular signalling, apoptosis, and cytoskeletal organisation pathways. Importantly, 50 proteins altered by MIA in embryonic and adult HPC were enriched in the NPD-relevant synaptic vesicle cycle. A persistently upregulated protein cluster formed a functional network involved in presynaptic signalling and proteins downregulated in embryos but upregulated in adults by MIA were correlated with observed social deficits. 49/50 genes encoding these proteins were significantly associated with NPD- and comorbidity-relevant traits in human phenome-wise association study data for psychiatric phenotypes. These findings highlight NPD-relevant targets for future study and early intervention in at-risk individuals. MIA-evoked changes in the neuroarchitecture of the NPD-relevant HPC and prefrontal cortex (PFC) of male and female mice highlighted sex- and region-specific alterations in dendritic and spine morphology, possibly underlining behavioural phenotypes.
To further investigate genetic risk factors of NPDs, I performed a study based on the implications of RBFOX1’s pleiotropic role in neuropsychiatric disorders and previous preclinical findings. Cytoplasmatic OE of RBFOX1, which affects the stability and translation of thousands of targets, was used to disseminate its role in morphology and behaviour. RBFOX1 OE affected dendritic length and branching in the male PFC and led to spine alterations in both PFC and HPC. Due to previously observed ASD-like endophenotypes in our Rbfox1 KO mice and the importance of gene × environment effects on NPD susceptibility, I probed the interaction of cytoplasmatic OE and a low-dose MIA on offspring. Both RBFOX1 OE alone and with MIA led to increased offspring loss during the perinatal period. Preliminary data suggested that RBFOX1 OE × MIA might increase anxiety- and anhedonia-like behaviours. Morphological changes in the adult male OE HPC and PFC suggested increased spine density and reduced dendritic complexity. A small post-mortem study in human dorsolateral PFC of older adults did not reveal significant effects of a common risk variant on RBFOX1 abundance.
To expand upon NPD genetic risks, I evaluated the effects of a homo- (KO) or heterozygous (HET) Diras2 deletion in a novel, neuron-specific mouse model. DIRAS2’s function is largely unknown, but it has been associated with ADHD in humans and neurodevelopment in vitro. In adult mice, there were subtle sex-specific effects on behaviour, i.e. more pronounced NPD-relevant deficits in males, in keeping with human data. KO mice had subtly improved cognitive performance, while HET mice exhibited behaviours in line with core ADHD symptoms, e.g. earning difficulties (females), response inhibition deficits and hyperactivity (males), suggesting Diras2 dose-sensitivity and sex-specificity. The morphological findings revealed multiple aberrations in dendritic and spine morphology in the adult PFC, HPC, and amygdala of HET males. KOs changes in spine and dendritic morphology were exclusively in the PFC and largely opposite to those in HETs and NPD-like phenotypes. Region- and genotype-specific expression changes in Diras2 and Diras1 were observed in six relevant brain regions of adult HET and KO females, also revealing differences in the survival and morphology regulator mTOR, which might underlie observed differences.
In conclusion, the effects of MIA and partial Diras2 knockdown resembled each other in core, NPD-associated behavioural and morphological phenotypes, while cytoplasmatic RBFOX1 OE and full Diras2 KO differed from those. My findings suggest complex dose- and sex-dependent relationships between these prenatal and genetic interventions, whose NPD-relevant influences might converge onto neurodevelopmental molecular pathways. An assessment of such putative overlap, based on available data from the MIA proteomic analyses of embryonic and adult HPC, suggested the three models might be linked via downstream targets, interactions, and upstream regulators. Future studies should disseminate both distinct and shared aspects of MIA, RBFOX1, and DIRAS2 relevant to NPDs and build upon these findings.
Bei 166 Patienten wurde eine perkutane transluminale Angioplastie (PTA) der A. carotis interna bzw. communis durchgeführt. 37/166 Patienten hatten symptomatische Stenosen (22,3 %). Der Stenosegrad lag bei 76,0 ± 9,9 % (57- 97 %). Bei 165/166 Patienten (99,4 %) wurde die PTA erfolgreich durchgeführt und der Stenosegrad auf 5,8 ± 21,2 % ( -73- 79 %) gesenkt. Perioperativ traten bei 8/166 Patienten neurologische Komplikationen auf (4,8 %), darunter fünf cerebrale Infarkte (3,0 %). Perioperative verstarben vier Patienten (2,4 %), zwei an Folgen einer Apoplexie, zwei an Folgen eines Myokardinfarktes. 152/166 Patienten wurden durchschnittlich 16 ± 18 Monate (ein Tag nach PTA bis maximal 87 Monaten) nachuntersucht. Bei 17 Patienten kam es zu Rezidivstenosen (10,2 %), drei davon zeigten neurologische Symptome, drei weitere bedurften einer wiederholten PTA (Re-PTA).
Die perioperativen Ergebnisse der vorgelegten Arbeit deckten sich weitgehend mit denen anderer Studien zur PTA 5, sowie zu denen bedeutender Studien zur Thrombendarteriektomie (TEA) 6. Diese Ergebnisse wurden durch die randomisierte CAVATAS- Studie
bestätigt, in der keine signifikanten Unterschiede bezüglich der Inzidenz neurologischer Symptome und der Mortalität der PTA und TEA nachgewiesen wurden [18]. Größer angelegte Studien hierzu werden zur Zeit in Europa und Nordamerika durchgeführt (CREST, SPACE). Rezidivstenosen scheinen häufiger nach PTAs aufzutreten, wobei Verbesserungen durch neue Techniken für die PTA und auch die TEA zu erwarten sind. Die klinische Bedeutung der Rezidivstenosen erwies sich bei der PTA als relativ unbedeutend.
Bei der Ermittlung von Risikofaktoren (Patientenalter; Stenoselänge; Kalzifikationen, Ulcerationen und Thromben in der Läsion) wurde festgestellt, dass bei der PTA andere Faktoren bedeutsam sind als bei der TEA. Zukünftig könnten Komplikationen vermieden werden, wenn Patienten aufgrund ihres Risikoprofils zur jeweils vorteilhafteren Behandlung zugewiesen würden.
Bei 91/166 Patienten wurde ein Embolieprotektionssystem verwandt. Es wurden verschiedene Methoden angewendet: distale Occlusion (PercuSurge™ System, n = 46), Filtersysteme (4 verschiedene Typen, n = 43) und proximale Occlusion (Arteriasystem, n = 2). Die Erfolgsrate lag bei 95,6 % und unterschied sich nicht signifikant innerhalb der einzelnen Systeme (PercuSurge™ System: 95,7 %; Filter: 95,3 %). Bei 67/91 Patienten wurde thrombotisches Material in den Filtern, bzw. im Aspirat festgestellt (73,6 %), wobei die Filter häufiger Partikel zurückhielten als das PercuSurge™ System (81,4 % versus 69,6 %). Bei vier Patienten kam es während der Protektion zu neurologischen Komplikationen, wobei in keinem dieser Fälle ein kausaler Zusammenhang zu den Protektionssysteme nachgewiesen werden konnte. Bei allen vier Patienten wurde thrombotisches Material nachgewiesen und entfernt. Die perioperative Inzidenz neurologischer Komplikationen lag bei 2,7 % (2/75 Patienten) ohne und bei 6,6 % (6/91 Patienten) mit Protektion. Die Mortalität lag bei 1,3 % (1/75 Patienten) ohne und bei 3,3 % (3/91 Patienten) mit Protektion. Diese Ergebnisse waren nicht signifikant. Die Ergebnisse der Filter und des PercuSurge™ Systems unterschieden sich ebenfalls nicht signifikant voneinander (PercuSurge™ System, Inzidenz neurologischer Komplikationen und Mortalität: 4,3 %; Filtersysteme, Inzidenz neurologischer Komplikationen: 9,3 %, Mortalität: 2,3 %). Während der PTA traten periprocedurale neurologische Symptome (PNS) bei Benutzung von Filtersystemen bei 8/43 Patienten (18,6 %), beim PercuSurge™ System bei 17/46 Patienten (37,0 %) auf.
Alle Protektionssysteme erwiesen sich in der vorliegenden Arbeit als praktikabel und für die klinische Routine geeignet. Der häufige Nachweis von Partikeln bestätigte den Nutzen der Protektion. In dieser Arbeit zeigte sich jedoch eine höhere Inzidenz neurologischer Komplikationen und Mortalität. Aufgrund der durchweg positiven Erfahrungen in vielen anderen Studien ist jedoch der klinische Nutzen der Protektion unzweifelhaft [70, 90, 132]. Kein signifikanter Unterschied konnte zwischen dem PercuSurge™ System den Filtersystemen festgestellt werden. Das PercuSurge™ System zeichnete eine geringere Inzidenz neurologischer Komplikationen und eine höhere technische Erfolgsrate aus, die Filter eine geringere Inzidenz von PNS und eine größere Effektivität beim Zurückhalten von thromboembolischen Partikeln.
Gerade in der Medizin besteht die Diagnostik nicht nur aus Zahlen und Werten, die Gesamtheit einer Diagnostik setzt sich häufig auch aus Bildern zusammen. Die bildgebende Diagnostik lässt nur wenig Spielraum für eine platzsparende Arbeitsweise, zu wichtig ist der wissenschaftliche Aussagewert feinster Details, seien es nun radiologische Aufnahmen, klinische Bilder oder Histologien. Moderne Computernetzwerke bieten eine geeignete Infrastruktur zum Austausch relevanter Daten in akzeptabler Geschwindigkeit bei guter Datenqualität. So ist die EDV nicht nur Datenverwalter, sondern ein aktives Hilfsmittel im medizinischen Alltagsgebrauch.
Diese Arbeit soll zeigen, welche technischen Voraussetzungen erfüllt sein müssen, um durch Einsatz digitaler Bilderfassungsgeräte eine optimale Datengrundlage für Forschung, Lehre und Dokumentation zu schaffen.
Die gezeigten Systeme sind grundsätzlich in statische „solid-state“ Systeme und dynamische „remote“-Systeme zu unterteilen.
Statische Systeme sind der asynchronen Telemedizin zuzuordnen und arbeiten nach dem store and forward Prinzip. Die Aufnahme ist nicht erweiterbar, in der Ferne wird zeitlich unabhängig die Diagnose gestellt. Dieses System bietet sich in strukturschwachen Regionen an.
Remotesysteme sind technisch sehr viel aufwendiger. Die sogenannte synchrone Telemedizin ist auch räumlich getrennt, findet jedoch zeitgleich statt. Das heißt, über eine Kamera und Datenleitung sind die Experten direkt mit dem Präparat oder dem Patienten verbunden. Aufgrund des technischen Aufwandes sind solche Systeme in der Regel in strukturstarken Regionen zu finden.
Beide Varianten messen sich bezüglich ihrer Validität an dem sogenannten „GoldStandard“, dem direkten Kontakt von Facharzt oder Pathologen zu Patient oder Präparat. Abhängig von der technischen Qualität haben sich beide telemedizinischen Prinzipien als in hohem Maße valide dargestellt.
Die Wahl der Kamera als zentrales Übersetzungsmedium von Bild in Daten setzt ein exaktes Studium der technischen Grundlagen voraus, wenn man bezüglich Auflösung und Lichtempfindlichkeit keine Kompromisse eingehen möchte, aber auch, wenn die physikalischen Grenzen, wie etwa die optische Auflösung eines Systems, die Notwendigkeit eines hochauflösendsten Systems nicht sinnvoll erscheinen lässt. Die Archivierung der Daten erfordert zudem Speicherplatz. Die Kompression von Bildern kann verlustfrei, aber auch verlustbehaftet stattfinden. Je nach Anwendungszweck gilt es daher abzuwägen, ob objektive Verlustfreiheit bei höherem Speicherbedarf oder wenig Platzbedarf bei einem eventuellen Qualitätsverlust gewünscht ist. Im hier gestellten Vergleich zwischen einem hochauflösenden Single-Shot System und einem Multishotaufbau hat sich gezeigt, dass die Qualität des Bildes bei einem hochauflösenden Einzelbild homogener ist, während das gekachelte Bild durch das Merging-Verfahren Belichtungs- und Schärfeunterschiede aufweist und die Bereiche der Bildteilzusammenführung erkennen lassen kann. Als Vorteil eines solchen Aufbaus ist sicherlich die technisch nahezu unbegrenzte Bildfläche zu nennen, da das aufgenommene Areal nicht von einem Einzelbild, sondern der Zahl der Segmente abhängt. Insofern haben beide Konzepte abhängig vom Einsatzanspruch ihre Existenzberechtigung.
Multilevel-Untersuchung des nitrinergen Systems bei affektiven Störungen und schizophrenen Psychosen

Das nitrinerge System und damit auch NOx als Neurotransmitter werden mit der Entstehung verschiedener psychischer Erkrankungen in Verbindung gebracht. Die genaue Rolle des Botenstoffs ist jedoch nicht ausreichend geklärt und auch die Frage, ob dieser als diagnostischer oder prädiktiver Biomarker nützlich sein könnte, ist unbeantwortet. In der vorliegenden Arbeit wurde folglich untersucht, ob es Unterschiede zwischen den Diagnosegruppen MDD, BIP, SCZ und der Kontrollgruppe bezüglich peripherer NOx- Konzentrationen gibt. Darüber hinaus wurden Unterschiede innerhalb der Diagnosegruppen im Krankheitsverlauf im Sinne von Phasenunterschieden mittels zweier Messzeitpunkte untersucht und analysiert, ob es Korrelationen mit genetischen Variationen in NOS-Genabschnitten gibt. Insgesamt wurden 185 Probanden in die Studie mitaufgenommen: 52 gesunde CTRL, 43 Patienten mit MDD, 41 Patienten mit BIP und 49 Patienten mit SCZ. Biochemische, genetische und klinische Daten wurden bei Aufnahme und Entlassung in der psychiatrischen Abteilung des Universitätsklinikums Frankfurt erhoben. Klinische Daten, die den Symptomverlauf 90 und die Erkrankungsschwere beurteilten, nutzten dazu standardisierte teilstrukturierte klinische Interviews. Biochemische Daten wurden mittels im Serum gemessener NOx- Spiegel quantifiziert. Bezüglich der Untersuchung der Risikogenvarianten wurden Probanden anhand des NOS1 ex1f-VNTR-Polymorphismus sowie SNPs in den Genen NOS1, NOS3 und NOS1AP genotypisiert. Bei Aufnahme wiesen SCZ-Patienten im Vergleich zu CTRL-, MDD- und BIP-Gruppen signifikant höhere NOx- Konzentrationen auf. Während NOx- Spiegel im Behandlungsverlauf bei MDD- und BIP-Patienten signifikant zunahmen, konnte dies bei SCZ-Patienten nicht beobachtet werden. Weiterhin konnte gezeigt werden, dass Patienten, deren depressive Beschwerden nicht relevant zurückgingen, bei Entlassung signifikant höhere NOx- Konzentrationen aufwiesen, was durch die Beobachtung einer signifikant positiven Korrelation zwischen NOx- Serumspiegeln und depressiven Symptomen bei Entlassung unterstützt wurde. Bei der genetischen Untersuchung der Daten fiel auf, dass homozygote Träger des kurzen VNTR-Allels signifikant erhöhte NOx- Konzentrationen besaßen. Diese Ergebnisse blieben bei jenen Trägern auch nach Entlassung signifikant. Insgesamt gibt es Hinweise darauf, dass erhöhte periphere NOx- Metabolitkonzentrationen mit einer Zunahme der Psychopathologie bzw. der Erkrankungsschwere einhergehen könnten, was möglicherweise auf den NOS1 ex1f-VNRT-Polymorphismus zurückzuführen ist. Außerdem zeigten zwei SNPs, welche beide im NOS1AP-Gen lokalisiert sind, bei BIP Patienten signifikant gesteigerte NOx- Werte. Die vorliegenden Ergebnisse deuten darauf hin, dass NO-Signalübertragung und NOS-Genotypen in der Pathogenese psychischer Erkrankungen eine Rolle spielen könnten. Ob diese Veränderungen allerdings kausal mit Krankheitsprozessen zu tun haben oder ob es eher Epiphänomene der Erkrankungen sind, kann mit dieser Studie nicht geklärt werden. Die Genvarianten könnten wiederum bei der Regulierung von peripheren NOx- Konzentrationen von Bedeutung sein. Die Arbeit liefert zudem Hinweise, die Verwendung von NOx als möglichen peripheren Biomarker weiter zu verfeinern und zu untersuchen. Zukünftige Studien, die die Wirksamkeit von NOx- modulierenden Pharmaka untersuchen, könnten davon profitieren, Diagnosegruppen nach Subgruppen einzuteilen, die sowohl NOS Risikogenvarianten als auch periphere NOx- Spiegel im Sinne eines Biomarker beachten.
Polyunsaturated fatty acids (PUFAs) play essential roles in mediating inflammation and its resolution. PUFA metabolites generated by the cytochrome P450 (CYP) - soluble epoxide hydrolase (sEH) axis are known to regulate macrophage activation/polarization but little is known about their role in the resolution of inflammation. Monocytes were isolated from murine bone marrow or human peripheral blood and differentiated to naïve macrophages (M0). Thereafter cells were polarized using LPS and IFNγ (M1), IL-4 (M2a), or TGFβ1 (M2c). Gene expression was analyzed by RNA sequencing, RT-qPCR and Western blotting. Phagocytosis of zymosan and oxo-LDL were also assessed in vitro. Zymosan-induced peritonitis combined with immune cell profiling was used to evaluate the resolution of inflammation in vivo. The expression of sEH was comparable in M0, M1 and M2a macrophages but markedly elevated in M2c polarized cells. The increase in sEH expression elicited by TGFβ relied on the TGFβ receptor ALK5 and the phosphorylation of SMAD2, which was able to bind to the sEH promoter. In macrophages lacking sEH, M2c polarization was incomplete and characterized by lower levels of pro-resolving phagocytosis associated receptors (Tlr2 and Mrc1), as well as higher levels of the pro-inflammatory markers; Nlrp3, IL-1β and TNFα. Fitting with the failure to upregulate phagocytosis associated receptors, the uptake of zymosan and ox-LDL was less efficient in M2c macrophages from sEH-/- mice. The latter animals also demonstrated a retarded resolution of inflammation (zymosan-induced peritonitis) in vivo with fewer resident macrophages and recruited macrophages. PUFA profile analysis indicated decreased sEH substrates e.g., 11, 12-EET, as well as increased sEH products e.g., 11, 12-DHET, indicating an increased sEH activity in M2c macrophages. Taken together, our data indicates that sEH expression is required for the effective M2c polarization of macrophages and thus the resolution of inflammation.

• It is important to distinguish acute provoked seizures due to autoimmune encephalitis from chronic unprovoked seizures due to autoimmune-associated epilepsy.
• Currently it is hardly possible in an individual AIE/ALE/RE patient to separate acute provoked seizures from chronic unprovoked seizures due to limitations in determining seizure outcomes, unclear time courses, potential causal interactions between both seizure origins, compartmentalized immune-inflammation, and a lack of licensed drugs to reliably resolve immune-inflammation in the brain parenchyma.
• This makes it hard to decide when to terminate ASMs and to counsel the individual patient regarding driving abilities and other behavioral restrictions and recommendations.
• Studies are urgently needed to define clinical and paraclinical biomarkers in a hypothesis-free, data-driven approach reliably predicting (or not) the development of AAE and the cognitive and behavioral outcome in the due course of an individual patient´s disease.
• These studies should be experimentally validated in suitable animal models.
Abstract
The current International League Against Epilepsy (ILAE) definition and classification guidelines for the first time introduced the category of immune-mediated focal epilepsy in addition to structural, genetic, infectious, and metabolic aetiologies. Moreover, the ILAE Autoimmunity and Inflammation Taskforce recently provided a conceptual framework for the distinction between acute “provoked” seizures in the acute phase of autoimmune encephalitis from chronic “unprovoked” seizures due to autoimmune-associated epilepsy. The first category predominately applies to those autoimmune encephalitis patients with autoantibodies against cell surface neural antigens, in whom autoantibodies are assumed to exert a direct ictogenic effect without overt structural damage. These patients do not exhibit enduring predisposition to seizures after the “acute phase” encephalitis, and thus do not fulfil the definition of epilepsy. The second category applies to those autoimmune encephalitis patients with autoantibodies against intracellular neural antigens and Rasmussen's encephalitis, in whom T cells are assumed to cause epileptogenic effects through immune-inflammation and overt structural damage. These patients do exhibit enduring predisposition to seizures after the “acute phase” of encephalitis and thus fulfil the definition of epilepsy. AAE may result from both, ongoing brain autoimmunity and associated structural brain damage according to the current ILAE definition and classification guideline. We here discuss the shortcomings and defaults of this concept and suggest an unbiased translationally validated and data-driven approach to predict in an individual encephalitis patient the propensity to develop (or not) AAE and the cognitive and behavioural outcome.
Despite antagonizing attempts from the tobacco industry, passive inhalation of tobacco smoke is known to be cancerogenic and toxic to human health for decades. Nonetheless, millions of non-smoking adults and children are still victims of second-hand smoke. Accumulation of particulate matter (PM) in confined spaces such as the car are particularly harmful due to high concentrations. We here aimed to analyze the specific effects of ventilation conditions in the setting of a car. By the use of the measuring platform TAPaC (tobacco-associated particulate matter emissions inside a car cabin), 3R4F reference cigarettes, Marlboro red, and Marlboro gold were smoked in a car interior with a volume of 3.709 m3. Seven different ventilation conditions (C1–C7) were analyzed. Under C1, all windows were closed. Under C2–C7, the car ventilation was turned on power level 2/4 with the air directed towards the windshield. Only the passenger side window was opened, where an outer placed fan could create an airstream speed of 15.9–17.4 km/h at one meter distance to simulate a driving car. C2: Window 10 cm opened. C3: Window 10 cm opened with the fan turned on. C4: Window half-opened. C5: Window half-opened with the fan turned on. C6: Window fully opened. C7: Window fully opened with the fan turned on. Cigarettes were remotely smoked by an automatic environmental tobacco smoke emitter and a cigarette smoking device. Depending on the ventilation condition the cigarettes emitted different mean PM concentrations after 10 min under condition C1 (PM10: 1272–1697 µg/m3, PM2.5: 1253–1659 µg/m3, PM1: 964–1263 µg/m3) under C2, C4, and C6 (PM10: 68.7–196.2 µg/m3, PM2.5: 68.2–194.7 µg/m3, PM1: 66.1–183.8 µg/m3) C3, C5, and C7 (PM10: 73.7–139 µg/m3, PM2.5: 72–137.9 µg/m3, PM1:68.9–131.9 µg/m3). Vehicle ventilation is insufficient to protect passengers from toxic second-hand smoke completely. Brand-specific variations of tobacco ingredients and mixtures markedly influence PM emissions under ventilation conditions. The most efficient ventilation mode to reduce PM exposure was achieved by opening the passenger´s window 10 cm and turning the onboard ventilation on power level 2/4. In-vehicle smoking should be banned to preserve innocent risk groups (e.g., children) from harm.
Die klinische Pfade zielten in erster Linie darauf ab, die Aufenthaltsdauer zu verkürzen und unnötige Kosten zu sparen, während die Qualität der Pflege erhalten blieb oder verbessert wurde. Bei der laparoskopischen Cholezystektomie gibt es keine ausreichenden Beweise für einen Einfluss auf postoperative Komplikationen.
In dieser retrospektiven Studie wurde die logistische Regression verwendet, um einen Neigungswert zu berechnen, und nach dem Abgleich werden 296 Patienten in beiden Gruppen im Hinblick auf postoperative Komplikationen unter Verwendung des Clavien-Dindo-Klassifizierungssystems als primäres Ziel analysiert. Darüber hinaus wurden sekundäre Ziele wie Aufenthaltsdauer, Einhaltung und Abweichung vom klinischen Pfad in Bezug auf die Entlassung von Patienten analysiert. Das relative Risiko des primären Ergebnisses wurde berechnet und mit dem E-Wert als Ansatz für Sensitivitätstests verglichen.
Aufgrund des obligatorischen Teil der klinischen Pfad bei den Patienten betrug die Compliance 100 Prozent. In 16% der Fälle trat eine Abweichung vom Pfad in Bezug auf die geplante Entlassung des Patienten am zweiten Tag nach der Operation auf. Nach Anpassung um potenzielle Faktoren beträgt das relative Risiko beim Vergleich der Clavien-Dindo-Komplikationsbewertung 0 versus 1-4 ist 1,64 (95% CI 0,87; 3,11), was nicht signifikant unterschiedlich ist (p = 0,127).
Nach Matching beträgt die Verweildauer 3,69 Tage ohne bzw. 3,26 Tage mit dem klinischen Pfad.
Vergleich zu bereits implementierter strukturierter Standardoperationsverfahren kann ein klinischer Pfad postoperative Komplikationen nicht reduzieren.
Dennoch betrachten wir unseren klinischen Pfad als ein äußerst wertvolles Instrument für die interdisziplinäre Verwaltung des Krankenhausaufenthalts des Patienten unter der Aufsicht eines erfahrenen Chirurgen.
Spezialwissen für SCALE

Volker Zickermann und Eric Helfrich sind bei der Exzellenzcluster-Initiative SCALE (Subcellular Architecture of Life) dabei und werden dort ihre Expertise einbringen. Das Spezialgebiet des einen ist ein Proteinkomplex in den Mitochondrien, den Kraftwerken der Zelle. Der andere sucht schwerpunktmäßig bisher unbekannte Naturstoffe, die die Basis für neue Antibiotika sein könnten.

Moderne Methoden der Künstlichen Intelligenz (KI) spielen in der Wissenschaft eine immer größere Rolle. Wie Forscher des Exzellenzclusters Cardio-Pulmonary Institute (CPI) KI in der Herzbildgebung nutzen, zeigte Professor Eike Nagel im Rahmen der Bürgeruniversität der Goethe-Universität. Er leitet das Institut für experimentelle und translationale kardiovaskuläre Bildgebung am Fachbereich Medizin und forscht an der Entwicklung verbesserter Behandlungsmöglichkeiten für Herz-Kreislauf-Erkrankungen. Mit dem Ziel, seine Forschung für alle Menschen zugänglich und verständlicher zu machen, lud Prof. Nagel interessierte Bürger*innen am 10. Mai in sein Institut ein.
Die TPTZ-Citrat-Methode für die Eisenbestimmung im Serum bzw. Plasma zeichnet sich durch verschiedene Vorteile gegenüber anderen handelsüblichen Methoden aus.
1. Der Eisennachweis wird nur zu 2% durch Kupfer falsch-positiv gestört.
2. Die Farbreaktion erfolgt in Citratpuffer bei einem pH von 2,1 bis 2,5. Dieses Milieu ist günstig für die Freisetzung von Eisen aus der Transferrinbindung und gleichzeitig optimal für die Farbentwicklung. Es verhindert in der Regel Proteinpräzipitation, so daß ohne Enteiweißung gemessen werden kann. Die Freisetzung von Eisen aus Hämoglobin wird minimiert. Die Automation dieses Eisentests ist unproblematisch.
3. Die TPTZ-Citrat-Methode ist kostengünstig im Vergleich zu handelsüblichen Tests. Auf Reagenzienbasis reduziert sich der Preis pro Analyse auf 1/4 bis 1/10.
[Abstract der Posterpräsentation] Aussagekraft der Tumormarker SCC und CA-125 bei Zervixkarzinom

Insgesamt 311 Stämme gramnegativer harnwegspathogener Enterobacteriaceen und Nonfermenter, davon 200 Isolate aus frischem Urin der täglichen Routine und 111 ausgewählte, bezüglich ihrer Identifikation problematische Keime aus der Stammsammlung des Zentrums der Hygiene, Frankfurt/Main, wurden mit den Systemen RAS-ID-Gramne9, und API 20 E bzw. NE, vergleichend getestet. Das RAS~ID-Gramne9-System benutzt 10 biochemische Reaktionen zur Identifizierung gramnegativer Bakterien sowie 10 Chemotherapeutika zur Resistenzbestimmung. Von den 200 Routinestämmen zeigten 196 (98%), von den 111 Stämmen aus der Stammsammlung 98 (88,3 %) Übereinstimmung. Die gute Übereinstimmung und die schnelle und einfache Handhabung läßt das RAS-ID-Gramne9-System für die Identifizierung harnwegs-pathogener Routinekeime als kostengünstige Alternative zu anderen aufwendigeren Identifizierungssystemen erscheinen.
Das Virus der Frühsommermeningoenzephalitis (FSME) und Borrelia burgdorferi als Erreger der Lyme-Borreliose sind die klinischbedeutsamsten durch Zecken übertragenen Infektionserreger in Europa. Der vorliegende Fall beschreibt eine serologisch gesicherte. Doppelinfektion mit dem FSME-Virus und Borellia burgdorferi bei einer 69jährigen deutschen Patientin nach einem Zeckenstich in einem österreichischen Endemiegebiet. Klinisch bestand zum Zeitpunkt der Krankenhausaufnahme eine ausgeprägte Somnolenz und ein hochgradiges Doppelbildsehen. Ein passive Immunisierung gegen FSME war postexpositionell erfolgt, konnte eine Infektion jedoch nicht verhindern. Eine Doppelinfektion durch beide Erreger wurde durch den serologischen Nachweis von spezifischen IgG und IgM Antikörpern gegen das FSME-Virus und im weiteren Verlauf auch gegen Borrelia burgdorferi im ELISA beziehungsweise im rekombinanten Immunoblot gesichert. Obwohl Doppelinfektionen durch die beiden genannten Erregerselten sind, sollten sie bei zeckenübertragenen Erkrankungen mit untypischem Verlauf in der Differentialdiagnose berücksichtigt werden.
Im vorliegenden Fall wird von einer Fehldiagnose auf der Grundlage eines falsch-reaktiven Anti-HCV-Tests und eines falsch-reaktiven HCV-Nukleinsäureamplifikationstests (NAT) berichtet, die bei einem 58-jährigen chirurgischen Oberarzt im Rahmen einer arbeitsmedizinischen Vorsorgeuntersuchung im krankenhauseigenen Labor gestellt wurde und zu einem knapp zweimonatigen Berufsverbot führte. Basis dieser Fehldiagnose war ein wiederholt schwach reaktiver HCV-Antikörper-ELISA, der mit einem Nukleinsäureamplifikationstest, der ebenfalls schwach positiv ausfiel, überprüft wurde. Ein Antikörperbestätigungs- bzw. Ergänzungstest (Immunoblot) wurde nicht durchgeführt. Die Fehldiagnose ist jedoch nicht durch einen Testfehler, sondern durch ein Missverständnis entstanden, indem beim Kliniker zwei Laborindizien zu einem Beweis aufsummiert wurden.
Die steigende Zahl von Pilzinfektionen, die Entwicklung und Einführung neuer anti-mykotischer Substanzen sowie die Möglichkeit der Resistenzentwicklung unter Therapie mit Antimykotikahaben in der Vergangenheit zu einem ständig wachsenden Bedarf an standardisierten Verfahren zur Empfindlichkeitstestung von pathogenen Pilzen geführt. Hierbei entstand unter anderem eine Vielzahl kommerzieller Testverfahren, bei denen mit Hilfe vorgefertigter Testkits eine einfache und schnelle Durchführung der Empfindlichkeitsprüfung erzielt werden soll. Eine dieser Methoden, welche in manchen Laboratorien in Deutschland angewendet wird, ist das so genannte LD 2Ring-Verfahren, welches auf dem Prinzip der Agardiffusion beruht unter Verwendung vorgefertigter, antimy-kotika-beschichteter Papierringe. In der vorliegenden Arbeit wird dieses Verfahren auf seine Reproduzierbarkeit bei der Testung von zehn Qualitätskontrollstämmen hin überprüft. Die Ergebnisse zeigen eine starke Schwankungsbreite und somit eine schlechte Reproduzierbarkeit, so dass dieses Verfahren zwar für die Bearbeitung von wissenschaftlichen Fragestellungen, nicht jedoch für die Routinetestung als geeignet angesehen werden kann. Des Weiteren erfolgte eine Untersuchung auf das Vor-liegen eines so genannten "minor error", man erhält für einen sensiblen Stamm das Ergebnis "resistent", "major error", man erhält für einen intermediären Stamm das Ergebnis "sensibel", und "very major error", man erhaält für einen resistenten Stamm das Ergebnis "sensibel". Hierbei kam es in 16,25% der untersuchten Fälle zum Vorliegen eines "minor errors". Ein "major error" wurde nicht beobachtet
Background: Novel treatments are needed to control refractory status epilepticus (SE). This study aimed to assess the potential effectiveness of fenfluramine (FFA) as an acute treatment option for SE. We present a summary of clinical cases where oral FFA was used in SE.
Methods: A case of an adult patient with Lennox–Gastaut syndrome (LGS) who was treated with FFA due to refractory SE is presented in detail. To identify studies that evaluated the use of FFA in SE, we performed a systematic literature search.
Results: Four case reports on the acute treatment with FFA of SE in children and adults with Dravet syndrome (DS) and LGS were available. We report in detail a 30-year-old woman with LGS of structural etiology, who presented with generalized tonic and dialeptic seizures manifesting at high frequencies without a return to clinical baseline constituting the diagnosis of SE. Treatment with anti-seizure medications up to lacosamide 600 mg/d, brivaracetam 300 mg/d, valproate 1,600 mg/d, and various benzodiazepines did not resolve the SE. Due to ongoing refractory SE and following an unremarkable echocardiography, treatment was initiated with FFA, with an initial dose of 10 mg/d (0.22 mg/kg body weight [bw]) and fast up-titration to 26 mg/d (0.58 mg/kg bw) within 10 days. Subsequently, the patient experienced a resolution of SE within 4 days, accompanied by a notable improvement in clinical presentation and regaining her mobility, walking with the assistance of physiotherapists. In the three cases reported in the literature, DS patients with SE were treated with FFA, and a cessation of SE was observed within a few days. No treatment-emergent adverse events were observed during FFA treatment in any of the four cases.
Conclusions: Based on the reported cases, FFA might be a promising option for the acute treatment of SE in patients with DS and LGS. Observational data show a decreased SE frequency while on FFA, suggesting a potentially preventive role of FFA in these populations.

Purpose: Seizures pose a significant burden in patients with primary and secondary brain tumors during the end-of-life period. A wide range of 6 to 56% of clinically observed epileptic seizures at the end of life has been reported. We aimed to analyse the incidence of epileptic seizures at the end of life in brain tumor patients more accurately using not only clinical but also electrophysiological findings.
Methods: This retrospective, single center study included brain tumor patients who died during the stay on the ward or within 7 days after discharge between 01/2015 and 08/2020. Clinical observation of seizures derived from the original medical records and EEG findings (within 45 days prior to death) were analyzed to determine the incidence of seizures in that period.
Results: Of the 68 eligible patients, 50 patients (73.5%) suffered from seizures within 45 days prior to death, of which n = 24 had a status epilepticus. The diagnosis of seizures/ status epilepticus was determined either by the presentation of clinical signs in 45 patients and if not, by the detection of a (possible) non-convulsive status epilepticus in the EEG of five patients.
Conclusion: In the presence of neurologically trained staff and with the frequent use of routine EEG, we were able to identify seizures and to distinguish status epilepticus from encephalopathy/ hypoactive delirium. We detected a higher incidence of seizures and status epilepticus at the end of life in neurooncological patients than previously reported.
Pericytes are capillary-associated mural cells involved in the maintenance and the stability of the vascular network. This thesis aims to investigate the role of pericytes in the heart in the context of ageing and disease. We highlight the malignant effects of the remodelling in the heart and stress the focus on the role of cardiac pericytes in this context. We show that ageing reduces pericyte coverage and that myocardial infarction (MI) causes an activation of these cells. Single-nuclei and single-cell RNA sequencing analysis of murine hearts further revealed that the expression of the Regulator of G-protein signalling 5 (Rgs5) is reduced in cardiac pericytes both in ageing and transiently at day 1 and day 3 after MI. The loss of RGS5 in pericytes drives an entropic state of these mural cells characterized by morphological changes, excessive extracellular deposition and enhanced Gaq mediated GPCR signalling. The deletion of RGS5 in pericytes causes cardiac systolic dysfunction, induces myocardial fibrosis, and drives the activation of cardiac fibroblasts in a TGFb-dependent manner. In conclusion, our results describe the importance of pericytes maintaining cardiac homeostasis, identify RGS5 as a key regulator of this process and propose pericytes as crucial mediators of cardiac fibrosis and possible therapeutic targets to prevent cardiovascular disease.
Oncogenic transformation of lung epithelial cells is a multistep process, frequently starting with the inactivation of tumour suppressors and subsequent development of activating mutations in proto-oncogenes, such as members of the PI3K or MAPK families. Cells undergoing transformation have to adjust to changes, including altered metabolic requirements. This is achieved, in part, by modulating the protein abundance of transcription factors. Here, we report that the ubiquitin carboxyl-terminal hydrolase 28 (USP28) enables oncogenic reprogramming by regulating the protein abundance of proto-oncogenes such as c-JUN, c-MYC, NOTCH and ∆NP63 at early stages of malignant transformation. USP28 levels are increased in cancer compared with in normal cells due to a feed-forward loop, driven by increased amounts of oncogenic transcription factors such as c-MYC and c-JUN. Irrespective of oncogenic driver, interference with USP28 abundance or activity suppresses growth and survival of transformed lung cells. Furthermore, inhibition of USP28 via a small-molecule inhibitor resets the proteome of transformed cells towards a ‘premalignant’ state, and its inhibition synergizes with clinically established compounds used to target EGFRL858R-, BRAFV600E- or PI3KH1047R-driven tumour cells. Targeting USP28 protein abundance at an early stage via inhibition of its activity is therefore a feasible strategy for the treatment of early-stage lung tumours, and the observed synergism with current standard-of-care inhibitors holds the potential for improved targeting of established tumours.
Background and objectives: Our study aimed at examining the long-time inflammatory effects of rheumatoid arthritis (RA) as chronic immune-mediated disease on pain sensation and neuropathy development compared to healthy subjects (HS).
Methods: We used the quantitative sensory testing (QST) protocol of the German Research Network on Neuropathic Pain and Electroencephalography (EEG)–based contact heat evoked potentials (CHEPs) before and after topical capsaicin application. We recruited 16 RA patients in remission or low disease activity state (mean age: 59.38 years [± 10.18]) and 16 healthy subjects (mean age: 56.69 years [± 8.92]).
Results: The application of capsaicin cream on the thigh provoked a stronger effect in HS for both mechanical and heat pain thresholds (MPT and HPT, resp.), according to the area under the receiver operation characteristic (AUROC) (HS: HPT: 0.8965, MPT: 0.7402; RA: HPT: 0.7012, MPT: 0.6113). We observed contrary effects regarding changes in CHEPs (HS: g*max = − 0.65; RA patients: g*max = 0.72).
Conclusion: As the overall effect of topical capsaicin application was higher in HS for QST, we suggest the existence of a sensitization of TRPV1 channels in RA patients caused by long-time chronical inflammation, despite a lack of clinical signs of inflammation due to adequate treatment. The effect in CHEPs probably uncovers neuropathic symptoms. The effect of topical capsaicin on HPTs and CHEPs can act as a marker for the extent of sensitization and the development of neuropathic symptoms. Further studies are needed to prove if our proposed method can act as a marker for the success of anti-inflammatory treatment.
The ligand-sensing transcription factor Nurr1 emerges as a promising therapeutic target for neurodegenerative pathologies but Nurr1 ligands for functional studies and therapeutic validation are lacking. Here pronounced Nurr1 modulation by statins for which clinically relevant neuroprotective effects are demonstrated, is reported. Several statins directly affect Nurr1 activity in cellular and cell-free settings with low micromolar to sub-micromolar potencies. Simvastatin as example exhibits anti-inflammatory effects in astrocytes, which are abrogated by Nurr1 knockdown. Differential gene expression analysis in native and Nurr1-silenced cells reveals strong proinflammatory effects of Nurr1 knockdown while simvastatin treatment induces several neuroprotective mechanisms via Nurr1 involving changes in inflammatory, metabolic and cell cycle gene expression. Further in vitro evaluation confirms reduced inflammatory response, improved glucose metabolism, and cell cycle inhibition of simvastatin-treated neuronal cells. These findings suggest Nurr1 involvement in the well-documented but mechanistically elusive neuroprotection by statins.
Die Ergebnisse der Studie und die Diversität der Datenbanken ist groß.
Für 12 Datenbanken wurde ein Punktesystem mit elf Items entworfen, um die Qualität der einzelnen Datenbanken zu objektivieren. Keine Datenbank konnte alle Bewertungskriterien erfüllen. Der insgesamt schlechte Punktedurchschnitt ist ein Indikator für die Mängel der aktuell verfügbaren Datenbanken. Außerdem konnten wir einen Qualitätsunterschied zwischen kostenpflichtigen und kostenfreien Datenbanken beweisen und mussten im Zuge dieser Ergebnisse die Frage stellen, ob kostenfreie Datenbanken überhaupt nützlich sind. Zwischen den kostenpflichtigen Datenbanken fallen die Qualitätsunterschiede weniger gravierend aus, wenngleich Stärken und Schwächen sich deutlich unterscheiden. Die häufigsten Wechselwirkungen wurden in allen Datenbanken mit großem Abstand zwischen rein psychiatrischen Interaktionspaaren erfasst. Dieses zeigt, wie wechselwirkungsreich Psychopharmaka sind und dass psychiatrische Patienten besonders vulnerabel sind. Die Nutzung digitaler Hilfsmittel scheint bei Betrachtung der hohen Anzahl ausgegebener Warnmeldungen unabdingbar zu sein, dennoch existiert große Uneinheitlichkeit bei der Bewertung der einzelnen Interaktionen. Die Vorstellung, dass zwei Kliniker bei Nutzung zweier unterschiedlicher Datenbanken zu völlig unterschiedlichen Empfehlungen kommen, fällt nicht schwer. Gleichzeitig könnte die Kooperation von Heilberuflern, die unterschiedliche Datenbanken verwenden, die Chance auf zusätzlichen Informationsgewinn und Austausch erhöhen, was im Umkehrschluss in einer erhöhten Arzneimitteltherapiesicherheit resultiert. In Studien konnte der positive Effekt interdisziplinärer Zusammenarbeit bereits bewiesen werden.
Zusammenfassend konnten umfangreiche Differenzen zwischen allen untersuchten Datenbanken aufgezeigt werden. Um den Anforderungen des klinischen Alltags zu genügen, müssen digitale Unterstützungssysteme weiterentwickelt werden.
Die „ideale Datenbank“ gibt es bisher nicht – das lässt sich durch unser Punktesystem beweisen. Um im klinischen Alltag Patientensicherheit zu gewährleisten ist die Nutzung einer einzelnen Datenbank bisher nicht ausreichend.
Die Gewährung der Patientensicherheit sollte unser oberstes Ziel sein und um dieses zu erreichen, bedarf es vieler Komponenten. Neben der Nutzung und vor allem Weiterentwicklung digitaler Unterstützungssysteme sollte auch der zwischenmenschliche Austausch weiter gefördert werden. Interdisziplinäre Zusammenarbeit im Sinne pharmazeutischer Dienstleistungen zur Medikationsanalyse könnten ein zusätzliches Instrument zur Vermeidung arzneimittelbezogener Probleme werden.
Zukünftig werden unsere Patienten am meisten von optimaler Nutzung weiterentwickelter Technologien, sowie wachsendem zwischenmenschlichem Austausch profitieren.
Human feline leukaemia virus subgroup C receptor-related proteins 1 and 2 (FLVCR1 and FLVCR2) are members of the major facilitator superfamily1. Their dysfunction is linked to several clinical disorders, including PCARP, HSAN and Fowler syndrome2,3,4,5,6,7. Earlier studies concluded that FLVCR1 may function as a haem exporter8,9,10,11,12, whereas FLVCR2 was suggested to act as a haem importer13, yet conclusive biochemical and detailed molecular evidence remained elusive for the function of both transporters14,15,16. Here, we show that FLVCR1 and FLVCR2 facilitate the transport of choline and ethanolamine across the plasma membrane, using a concentration-driven substrate translocation process. Through structural and computational analyses, we have identified distinct conformational states of FLVCRs and unravelled the coordination chemistry underlying their substrate interactions. Fully conserved tryptophan and tyrosine residues form the binding pocket of both transporters and confer selectivity for choline and ethanolamine through cation–π interactions. Our findings clarify the mechanisms of choline and ethanolamine transport by FLVCR1 and FLVCR2, enhance our comprehension of disease-associated mutations that interfere with these vital processes and shed light on the conformational dynamics of these major facilitator superfamily proteins during the transport cycle.
Background: Despite advances in treatment of patients with non-small cell lung cancer, carriers of certain genetic alterations are prone to failure. One such factor frequently mutated, is the tumor suppressor PTEN. These tumors are supposed to be more resistant to radiation, chemo- and immunotherapy.
Results: We demonstrate that loss of PTEN led to altered expression of transcriptional programs which directly regulate therapy resistance, resulting in establishment of radiation resistance. While PTEN-deficient tumor cells were not dependent on DNA-PK for IR resistance nor activated ATR during IR, they showed a significant dependence for the DNA damage kinase ATM. Pharmacologic inhibition of ATM, via KU-60019 and AZD1390 at non-toxic doses, restored and even synergized with IR in PTEN-deficient human and murine NSCLC cells as well in a multicellular organotypic ex vivo tumor model.
Conclusion: PTEN tumors are addicted to ATM to detect and repair radiation induced DNA damage. This creates an exploitable bottleneck. At least in cellulo and ex vivo we show that low concentration of ATM inhibitor is able to synergise with IR to treat PTEN-deficient tumors in genetically well-defined IR resistant lung cancer models.

We conducted a systematic review investigating the efficacy and tolerability of adrenocorticotropic hormone (ACTH) and corticosteroids in children with epilepsies other than infantile epileptic spasm syndrome (IESS) that are resistant to anti-seizure medication (ASM). We included retrospective and prospective studies reporting on more than five patients and with clear case definitions and descriptions of treatment and outcome measures. We searched multiple databases and registries, and we assessed the risk of bias in the selected studies using a questionnaire based on published templates. Results were summarized with meta-analyses that pooled logit-transformed proportions or rates. Subgroup analyses and univariable and multivariable meta-regressions were performed to examine the influence of covariates. We included 38 studies (2 controlled and 5 uncontrolled prospective; 31 retrospective) involving 1152 patients. Meta-analysis of aggregate data for the primary outcomes of seizure response and reduction of electroencephalography (EEG) spikes at the end of treatment yielded pooled proportions (PPs) of 0.60 (95% confidence interval [CI] 0.52–0.67) and 0.56 (95% CI 0.43–0.68). The relapse rate was high (PP 0.33, 95% CI 0.27–0.40). Group analyses and meta-regression showed a small benefit of ACTH and no difference between all other corticosteroids, a slightly better effect in electric status epilepticus in slow sleep (ESES) and a weaker effect in patients with cognitive impairment and “symptomatic” etiology. Obesity and Cushing's syndrome were the most common adverse effects, occurring more frequently in trials addressing continuous ACTH (PP 0.73, 95% CI 0.48–0.89) or corticosteroids (PP 0.72, 95% CI 0.54–0.85) than intermittent intravenous or oral corticosteroid administration (PP 0.05, 95% CI 0.02–0.10). The validity of these results is limited by the high risk of bias in most included studies and large heterogeneity among study results. This report was registered under International Prospective Register of Systematic Reviews (PROSPERO) number CRD42022313846. We received no financial support.

Diese Arbeit hatte das Ziel, die Größe einer DVT-Aufnahme (FOV) mit der Größe der durch die Indikationsstellung definierten Region (ROI) zu vergleichen. Durch eine speziell dafür entwickelte Software sollten Messungen in den Datensätzen ermöglicht werden. Die dazu verwendeten 332 Datensätze wurden zufallsverteilt aus den mit einem Orthophos SL-3D DVT-Gerät der Firma Dentsply Sirona in der Poliklinik für zahnärztliche Chirurgie und Implantologie des ZZMK (Carolinum) in Frankfurt angefertigten Röntgenaufnahmen selektiert.
Es wurde die Auswertungssoftware ExRoi entwickelt, mit der die Werte des axialen Durchmessers, die Höhe (vertikale Dimension) sowie die Distanz der Mittelpunkte von FOV und ROI direkt in den Datensätzen bestimmt werden konnten. Zusätzlich wurde festgehalten, welche rechtfertigenden Indikationen gestellt und welche Auflösungsmodi verwendet wurden.
Die Stichprobe bestand aus Aufnahmen mit einem axialen Durchmesser von 8 cm [VOL 1] (n= 76, entsprechend 46,39%), 5 cm Durchmesser [VOL 2] (n = 102, entsprechend 30,72%) und 11 cm Durchmesser [VOL 3] (n= 154, entsprechend 22,89%). 95,18% der Aufnahmen wurden im HD-Modus mit laut Herstellerangaben vier Mal so vielen Aufnahmen im Vergleich zum SD-Modus angefertigt. Hauptindikationen waren Implantat Planung (45,1%) und Planungen komplizierter Zahn-Extraktionen (25,5%).
Die Messungen zum Vergleich des axialen Durchmessers zeigten, dass bei Verwendung des VOL 2 die ROI im Mittel den größten Anteil der FOV nutzt (78,52 %), den kleinsten Anteil nutzt durchschnittlich VOL 1 (56,04 %). Dazwischen liegt VOL 3 (69,12 %). In der Vertikalen nutzt die ROI von VOL 3 mittelwertig den größten Anteil der FOV (81,87 %), den kleinsten Mittelwert hat VOL 1 (58,76 %). VOL 2 liegt zwischen diesen Werten (64,47 %).
In allen Fällen war das FOV größer als die ROI und die ROI lag im Bereich des gewählten FOV.
Die Mittelpunkte von FOV und ROI lagen im Mittel in der axialen Ebene in Abhängigkeit vom gewählten Volumen um rund 9-13 mm auseinander, in der coronalen und sagittalen Ebene um rund 5-6mm.
Aus diesen Ergebnissen kann für das verwendete Gerät eine gute Trefferquote für die ROI abgeleitet werden. Höhe und Durchmesser des FOV hätten in den meisten Fällen kleiner gewählt werden können, liegen aber angesichts der vorhandenen Auswahl-Optionen des Röntgengeräts zur Dimensionierung der Volumina in einem akzeptablen Bereich.
Background: Epilepsy surgery is an established treatment for drug-resistant focal epilepsy (DRFE) that results in seizure freedom in about 60% of patients. Correctly identifying an epileptogenic lesion in magnetic resonance imaging (MRI) is challenging but highly relevant since it improves the likelihood of being referred for presurgical diagnosis. The epileptogenic lesion’s etiology directly relates to the surgical intervention’s indication and outcome. Therefore, it is vital to correctly identify epileptogenic lesions and their etiology presurgically.
Methods: We compared the final histopathological diagnoses of all patients with DRFE undergoing epilepsy surgery at our center between 2015 and 2021 with their MRI diagnoses before and after presurgical diagnosis at our epilepsy center, including MRI evaluations by expert epilepsy neuroradiologists. Additionally, we analyzed the outcome of different subgroups.
Results: This study included 132 patients. The discordance between histopathology and MRI diagnoses significantly decreased from 61.3% for non-expert MRI evaluations (NEMRIs) to 22.1% for epilepsy center MRI evaluations (ECMRIs; p < 0.0001). The MRI-sensitivity improved significantly from 68.6% for NEMRIs to 97.7% for ECMRIs (p < 0.0001). Identifying focal cortical dysplasia (FCD) and amygdala dysplasia was the most challenging for both subgroups. 65.5% of patients with negative NEMRI were seizure-free 12 months postoperatively, no patient with negative ECMRI achieved seizure-freedom. The mean duration of epilepsy until surgical intervention was 13.6 years in patients with an initial negative NEMRI and 9.5 years in patients with a recognized lesion in NEMRI.
Conclusions: This study provides evidence that for patients with DRFE—especially those with initial negative findings in a non-expert MRI—an early consultation at an epilepsy center, including an ECMRI, is important for identifying candidates for epilepsy surgery. NEMRI-negative findings preoperatively do not preclude seizure freedom postoperatively. Therefore, patients with DRFE that remain MRI-negative after initial NEMRI should be referred to an epilepsy center for presurgical evaluation. Nonreferral based on NEMRI negativity may harm such patients and delay surgical intervention. However, ECMRI-negative patients have a reduced chance of becoming seizure-free after epilepsy surgery. Further improvements in MRI technique and evaluation are needed and should be directed towards improving sensitivity for FCDs and amygdala dysplasias.
Background: Patients with epilepsy often require a specialized treatment, which may differ because of the responsibility of the federal states for healthcare policy in Germany.
Objective: State-specific differences in healthcare structures based on inpatient hospital cases of epilepsy patients between 2000 and 2020 in relation to specialized treatment offers.
Material and methods: The inpatient hospital cases of the German federal states were evaluated using the Friedman test and time series trend analysis. A state-specific inpatient undertreatment or overtreatment of inpatient hospital cases outside the registered state was analyzed by comparing residence-related and treatment site-related case numbers with a threshold of ±5%.
Results: After age adjustment, significantly more inpatient cases were found in the “new states” compared to the “old states” (p < 0.001); the highest number of cases nationwide was found in Saarland with 224.8 ± 11.5 cases per 100,000 inhabitants. The trend analysis showed an increase in cases until the end of 2016 with a trend reversal from 2017 and a further significant decrease in hospital cases in the COVID year 2020. A relative inpatient undertreatment was shown for Brandenburg, Lower Saxony, Rhineland-Palatinate, Saxony-Anhalt, Schleswig-Holstein and Thuringia. Additional, possibly compensatory, inpatient care was found for all city states (Hamburg, Bremen and Berlin) and Baden-Wuerttemberg. In federal states with a relative inpatient undertreatment and/or high inpatient hospital case numbers, there was often a lower availability of specialized epilepsy centers, specialized outpatient clinics and epilepsy outpatient clinics.
Conclusion: In Germany there are state-specific differences in the structure of care, with higher inpatient hospital care in the “new states” and Saarland. In addition, there were federal states with disproportionately higher treatment of patients not registered in this federal state. A potential influencing factor may be the availability of centers with specialized treatment for epilepsy patients.
