Each lifecycle of the Hepatitis C virus (HCV) produces structural and non-structural (NS) proteins in equimolar. Structural proteins were either assembled or degraded by host proteolysis systems, while NS proteins remain inside the host cells and don’t accumulate. Therefore, they must be degraded. Here, NS3 and NS5A half-lives were quantified in the presence of autolysosome and proteasome different modulators. Inhibitors of both systems increased the half-life, while inducers decreased the half-life. Furthermore, polyubiquitination of NS3 and NS5A was observed. Additionally, their intracellular co-localization with autolysosome (LAMP2) and proteasome (PSMB5) was observed, and inhibitors of both systems increased the degree of co-localization. A better understanding of NS protein degradation might help to improve medical interventions during HCV infections in the future.
Insulin resistance and working memory exploring the role of blood glucose levels and lifestyle

vIntroduction: Type 2 diabetes mellitus and dementia are among the leading causes for reduced quality of life and life expectancy worldwide and often occur comorbidly. Both diseases are linked by altered insulin signaling. Lifestyle factors and blood glucose monitoring play an essential role in the prevention and treatment of type 2 diabetes. So far, a relationship between blood glucose levels, lifestyle, and cognitive performance – a main symptom of dementia - has mainly been established in laboratory settings which reduces its ecological validity.
Objectives: This study uses ambulatory assessment and continuous glucose monitoring to explore the link between blood glucose levels, lifestyle and working memory in an ecological setting. We hypothesize that glycemic variations affect working memory performance in daily life. Second, we hypothesize that a high variance in blood glucose levels has a higher impact on working memory in insulin resistant participants. With this study, we aim to expand the knowledge on the relationship of insulin resistance and cognitive performance from the laboratory setting to everyday life.
Methods: This prospective, exploratory study will include 80 subjects with insulin resistance and 80 healthy controls. At baseline, blood indicators of insulin resistance will be measured to determine group assignment. Our ambulatory assessment includes smartphone-based sampling and sensor-based assessment. Therefore, cognitive performance will be recorded over three consecutive days using a smartphone. Four times a day, a numerical working memory task is prompted by signal-based alarms on the smartphone. Blood glucose levels are recorded in parallel by continuous glucose monitoring. In addition, lifestyle factors such as diet ad physical activity are examined. Diet is assessed by 24-h dietary protocols and movement acceleration by accelerometery.
Multilevel modelling will be used to map the relationship between blood glucose levels and working memory at the within- and between-person level. Diet and exercise are included in the analyses as additional predictors.
Results: Data collection started in March 2021 and is ongoing. Up to now, 40 insulin resistant participants and 36 healthy controls have been measured. Our preliminary results indicate a positive association between blood glucose levels and working memory performance at the within-person level (estimate = .48, 95% CI [.07, .89], p =0.022). At the between-person level the analysis revealed an inverse association between blood glucose levels and working memory performance (estimate = -.45, 95 % CI [-.86 - -.05], p = 0.029).
Conclusion: Our preliminary results are in line with studies showing that an acute rise in blood glucose levels leads to short-term improvements, while stable glucose profiles are beneficial in the long term. This might expand the understanding of the impact of insulin resistance on working memory and represent a target for early interventions. Our preliminary analysis needs to be repeated in our final dataset to confirm our results.
The multistep PROTAC (PROteolysis TArgeting Chimeras) degradation process poses challenges for their rational development, as rate limiting steps determining PROTAC efficiency remain largely unknown. Moreover, the slow throughput of currently used endpoint assays does not allow the comprehensive analysis of larger series of PROTACs. Here we developed cell-based assays using NanoLuciferase and HaloTags, that allow measuring PROTAC induced degradation and ternary complex formation kinetics and stability in cells. Using PROTACs developed for degradation of WDR5, the characterization of the mode of action of these PROTACs in the early degradation cascade revealed a key role of ternary complex formation and stability. Comparing a series of ternary complex crystal structures highlighted the importance of an efficient E3-target interface for ternary complex stability. The developed assays outline a strategy for the rational optimization of PROTACs using a series of live cell assays monitoring key steps of the early PROTAC induced degradation pathway.
Significance The multistep PROTAC induced degradation process of a POI poses a significant challenge for the rational design of these bifunctional small molecules as critical steps that limit PROTAC efficacy cannot be easily assayed at required throughput. In addition, the cellular location of the POI may pose additional challenges as some cellular compartments, such as the nucleus, may not be easily reached by PROTAC molecules and the targeted E3 ligases may not be present in this cellular compartment. We propose therefore a comprehensive assay panel for PROTACs evaluation in cellular environments using a sensor system that allows continuous monitoring of the protein levels of the endogenous POI. We developed a cell line expressing WDR5 from its endogenous locus in fusion with a small sequence tag (HiBIT) that can be reconstituted to functional NanoLuciferase (NLuc). This system allowed continuous monitoring of endogenous WDR5 levels in cells and together with HaloTag system also the continuous monitoring of ternary complex (E3, WDR5 and PROTAC) formation. As this assay can be run at high throughput, we used this versatile system monitoring three diverse chemical series of WDR5 PROTACs that markedly differ in their degradation properties. Monitoring cell penetration, binary complex formation (PROTAC-WDR5 and PROTAC-VHL) as well as ternary complex formation we found that PROTAC efficiency highly correlated with synergy of ternary complex formation in cells. This study represents a first data set on diverse PROTACs studying this property in cellulo and it outlines a strategy for the rational optimization of PROTACs. It also provided kinetic data on ternary complex assembly and dissociation that may serve as a benchmark for future studies utilizing also kinetic properties for PROTAC development. Comparative structural studies revealed larger PROTAC mediated interaction surfaces for PROTACs that efficiently formed ternary complexes highlighting the utility of structure based optimization of PROTAC induced ternary complexes in the development process.
5-iodotubercidin sensitizes cells to RIPK1-dependent necroptosis by interfering with NFκB signaling

Receptor-interacting protein kinases (RIPK) −1 and −3 are master regulators of cell fate decisions in response to diverse stimuli and are subjected to multiple checkpoint controls. Earlier studies have established the presence of distinct IKK1/2 and p38/MK2-dependent checkpoints which suppress RIPK1 activation by directly phosphorylating it at different residues. In the present study, we investigated TNF-induced death in MAPK-activated protein kinase 2 (MK2)-deficient cells and show that MK2-deficiency or inactivation predominantly results in necroptotic cell death, even in the absence of caspase inhibition. While MK2-deficient cells can be rescued from necroptosis by RIPK1 inhibitors, RIPK3 inhibition seems to revert the process triggering apoptosis. To understand the mechanism of this necroptosis switch, we screened a 149-compound kinase inhibitor library for compounds which preferentially sensitize MK2-deficient MEFs to TNF-induced cell death. The most potent inhibitor identified was 5-Iodotubericidin, an adenosine analogue acting as adenosine kinase and protein kinase inhibitor. 5-ITu also potentiated LPS-induced necroptosis when combined with MK2 inhibition in RAW264.7 macrophages. Further mechanistic studies revealed that 5-Iodotubericidin induces RIPK1-dependent necroptosis in the absence of MK2 activity by suppressing IKK signaling. The identification of this role for the multitarget kinase inhibitor 5-ITu in TNF-, LPS- and chemotherapeutics-induced necroptosis will have potential implications in RIPK1-targeted therapies.
Objectives: Within the tertiary-case database, the authors tested for differences in long-term continence rates (≥ 12 months) between prostate cancer patients with extraprostatic vs. organ-confined disease who underwent Robotic-Assisted Radical Prostatectomy (RARP).
Method: In the institutional tertiary-care database the authors identified prostate cancer patients who underwent RARP between 01/2014 and 01/2021. The cohort was divided into two groups based on tumor extension in the final RARP specimen: patients with extraprostatic (pT3/4) vs. organ-confined (pT2) disease. Additionally, the authors conducted subgroup analyses within both the extraprostatic and organ-confined disease groups to compare continence rates before and after the implementation of the new surgical technique, which included Full Functional-Length Urethra preservation (FFLU) and Neurovascular Structure-Adjacent Frozen-Section Examination (NeuroSAFE). Multivariable logistic regression models addressing long-term continence were used.
Results: Overall, the authors identified 201 study patients of whom 75 (37 %) exhibited extraprostatic and 126 (63 %) organ-confined disease. There was no significant difference in long-term continence rates between patients with extraprostatic and organ-confined disease (77 vs. 83 %; p = 0.3). Following the implementation of FFLU+ NeuroSAFE, there was an overall improvement in continence from 67 % to 89 % (Δ = 22 %; p < 0.001). No difference in the magnitude of improved continence rates between extraprostatic vs. organ-confined disease was observed (Δ = 22 % vs. Δ = 20 %). In multivariable logistic regression models, no difference between extraprostatic vs. organ-confined disease in long-term continence was observed (Odds Ratio: 0.91; p = 0.85).
Conclusion: In this tertiary-based institutional study, patients with extraprostatic and organ-confined prostate cancer exhibited comparable long-term continence rates.
Single nucleotide polymorphisms (SNPs) in the ADGRL3 gene have been significantly associated with the development of ADHD, the aetiology of which remains poorly understood. The rs1397547 SNP has additionally been associated with significantly altered ADGRL3 transcription. We therefore generated iPSCs from two wild type ADHD patients, and two ADHD patients heterozygous for the risk SNP. With this resource we aim to facilitate further investigation into the complex and heterogenous pathology of ADHD. Furthermore, we demonstrate the feasibility of using magnetic activated cell sorting to allow the unbiased selection of fully reprogrammed iPSCs.
Background: Pancreatic ductal adenocarcinoma remains one of the most lethal malignancies, with few treatment options. NAPOLI 3 aimed to compare the efficacy and safety of NALIRIFOX versus nab-paclitaxel and gemcitabine as first-line therapy for metastatic pancreatic ductal adenocarcinoma (mPDAC).
Methods: NAPOLI 3 was a randomised, open-label, phase 3 study conducted at 187 community and academic sites in 18 countries worldwide across Europe, North America, South America, Asia, and Australia. Patients with mPDAC and Eastern Cooperative Oncology Group performance status score 0 or 1 were randomly assigned (1:1) to receive NALIRIFOX (liposomal irinotecan 50 mg/m2, oxaliplatin 60 mg/m2, leucovorin 400 mg/m2, and fluorouracil 2400 mg/m2, administered sequentially as a continuous intravenous infusion over 46 h) on days 1 and 15 of a 28-day cycle or nab-paclitaxel 125 mg/m2 and gemcitabine 1000 mg/m2, administered intravenously, on days 1, 8, and 15 of a 28-day cycle. Balanced block randomisation was stratified by geographical region, performance status, and liver metastases, managed through an interactive web response system. The primary endpoint was overall survival in the intention-to-treat population, evaluated when at least 543 events were observed across the two treatment groups. Safety was evaluated in all patients who received at least one dose of study treatment. This completed trial is registered with ClinicalTrials.gov, NCT04083235.
Findings: Between Feb 19, 2020 and Aug 17, 2021, 770 patients were randomly assigned (NALIRIFOX, 383; nab-paclitaxel–gemcitabine, 387; median follow-up 16·1 months [IQR 13·4–19·1]). Median overall survival was 11·1 months (95% CI 10·0–12·1) with NALIRIFOX versus 9·2 months (8·3–10·6) with nab-paclitaxel–gemcitabine (hazard ratio 0·83; 95% CI 0·70–0·99; p=0·036). Grade 3 or higher treatment-emergent adverse events occurred in 322 (87%) of 370 patients receiving NALIRIFOX and 326 (86%) of 379 patients receiving nab-paclitaxel–gemcitabine; treatment-related deaths occurred in six (2%) patients in the NALIRIFOX group and eight (2%) patients in the nab-paclitaxel–gemcitabine group.
Interpretation: Our findings support use of the NALIRIFOX regimen as a possible reference regimen for first-line treatment of mPDAC.
Substantial progress in the field of neuroscience has been made from anaesthetized preparations. Ketamine is one of the most used drugs in electrophysiology studies, but how ketamine affects neuronal responses is poorly understood. Here, we used in vivo electrophysiology and computational modelling to study how the auditory cortex of bats responds to vocalisations under anaesthesia and in wakefulness. In wakefulness, acoustic context increases neuronal discrimination of natural sounds. Neuron models predicted that ketamine affects the contextual discrimination of sounds regardless of the type of context heard by the animals (echolocation or communication sounds). However, empirical evidence showed that the predicted effect of ketamine occurs only if the acoustic context consists of low-pitched sounds (e.g., communication calls in bats). Using the empirical data, we updated the naïve models to show that differential effects of ketamine on cortical responses can be mediated by unbalanced changes in the firing rate of feedforward inputs to cortex, and changes in the depression of thalamo-cortical synaptic receptors. Combined, our findings obtained in vivo and in silico reveal the effects and mechanisms by which ketamine affects cortical responses to vocalisations.
The MICOS complex subunit MIC13 is essential for mitochondrial cristae organization. Mutations in MIC13 cause severe mitochondrial hepato-encephalopathy displaying defective cristae morphology and loss of the MIC10-subcomplex. Here we identified stomatin-like protein 2 (SLP2) as an interacting partner of MIC13 and decipher a critical role of SLP2 as an auxiliary MICOS subunit, modulating cristae morphology. SLP2 provides a large interaction hub for MICOS subunits and loss of SLP2 leads to drastic alterations in cristae morphology. Double deletion of SLP2 and MIC13 showed reduced assembly of core MICOS subunit, MIC60 into MICOS and dispersion of MIC60-specific puncta, demonstrating a critical role of SLP2-MIC13 in MICOS assembly and crista junction (CJ) formation. We further identified that the mitochondrial i-AAA protease YME1L in coordination either with MIC13 or SLP2 differentially regulates MICOS assembly pathways thereby interlinking MIC13-specific or scaffolding-specific role of SLP2 with quality control and assembly of the MICOS complex. YME1L- depletion in MIC13 KO could restore MIC10-subcomplex and reform the nascent CJ. Taken together, we propose ‘seeder’ model for MICOS assembly and CJ formation, where SLP2- MIC13 seed the assembly of MIC60 into MICOS complex and promote the formation of CJ by regulating the quality and stability of MIC10-subcomplex.
RBFOX1 is a highly pleiotropic gene that contributes to several psychiatric and neurodevelopmental disorders. Both rare and common variants in RBFOX1 have been associated with several psychiatric conditions, but the mechanisms underlying the pleiotropic effects of RBFOX1 are not yet understood. Here we found that, in zebrafish, rbfox1 is expressed in spinal cord, mid- and hindbrain during developmental stages. In adults, expression is restricted to specific areas of the brain, including telencephalic and diencephalic regions with an important role in receiving and processing sensory information and in directing behaviour. To investigate the effect of rbfox1 deficiency on behaviour, we used rbfox1sa15940, a rbfox1 loss-of-function line. We found that rbfox1sa15940 mutants present hyperactivity, thigmotaxis, decreased freezing behaviour and altered social behaviour. We repeated these behavioural tests in a second rbfox1 loss-of-function line with a different genetic background, rbfox1del19, and found that rbfox1 deficiency affects behaviour similarly in this line, although there were some differences. rbfox1del19 mutants present similar thigmotaxis, but stronger alterations in social behaviour and lower levels of hyperactivity than rbfox1sa15940 fish. Taken together, these results suggest that rbfox1 deficiency leads to multiple behavioural changes in zebrafish that might be modulated by environmental, epigenetic and genetic background effects, and that resemble phenotypic alterations present in Rbfox1-deficient mice and in patients with different psychiatric conditions. Our study thus highlights the evolutionary conservation of rbfox1 function in behaviour and paves the way to further investigate the mechanisms underlying rbfox1 pleiotropy on the onset of neurodevelopmental and psychiatric disorders.
In the life sciences, there is an ongoing discussion about a perceived ‘reproducibility crisis’. However, it remains unclear to which extent the perceived lack of reproducibility is the consequence of issues that can be tackled and to which extent it may be the consequence of unrealistic expectations of the technical level of reproducibility. Large-scale, multi-institutional experimental replication studies are very cost- and time-intensive. This Perspective suggests an alternative, complementary approach: meta-research using sociological and philosophical methodologies to examine researcher trust in data. An improved understanding of the criteria used by researchers to judge data reliability will provide crucial, initial evidence on the actual scale of the reproducibility crisis and on measures to tackle it.
Classical molecular dynamics (MD) simulations provide unmatched spatial and time resolution of protein structure and function. However, accuracy of MD simulations often depends on the quality of force field parameters and the time scale of sampling. Another limitation of conventional MD simulations is that the protonation states of titratable amino acid residues remain fixed during simulations, even though protonation state changes coupled to conformational dynamics are central to protein function. Due to the uncertainty in selecting protonation states, classical MD simulations are sometimes performed with all amino acids modeled in their standard charged states at pH 7. Here we performed and analyzed classical MD simulations on high-resolution cryo-EM structures of two membrane proteins that transfer protons by catalyzing protonation/deprotonation reactions. In simulations performed with amino acids modeled in their standard protonation state the structure diverges far from its starting conformation. In comparison, MD simulations performed with pre-determined protonation states of amino acid residues reproduce the structural conformation, protein hydration, and protein-water and protein-protein interactions of the structure much better. The results suggest it is crucial to perform basic protonation state calculations, especially on structures where protonation changes play an important functional role, prior to launching any MD simulations. Furthermore, the combined approach of protonation state prediction and MD simulations can provide valuable information on the charge states of amino acids in the cryo-EM sample. Even though accurate prediction of protonation states currently remains a challenge, we introduce an approach of combining pKa prediction with cryo-EM density map analysis that helps in improving not only the protonation state predictions, but also the atomic modeling of density data.
The present study aims to report the currently available epidemiology of focal onset seizures in children aged >1 month to 4 years with the help of a literature review. The terms ‘seizure*’ OR ‘epilepsy’ combined with pediatric and epidemiology terms were used to search Embase, PubMed, and Web of Science up to November 16, 2021. Due to the scarcity of epidemiology data on focal onset seizures, the incidence and prevalence were estimated using the proportion of focal onset seizures in epilepsy patients from the most recently published articles. The estimated annual incidence per 100,000 children of focal onset seizures in children of 0–4 years of age ranged from 25.1 (95 % confidence interval [CI] 18.9–32.7) in the United Kingdom to 111.8 in the United States. The estimated period prevalence of focal onset seizures in children 0–4 years of age ranged from 0.15 % (99 % CI 0.13–0.18) in Canada to 0.61 % in the United States. Neurodevelopmental outcomes and psychiatric disorders were the most commonly reported comorbidities in children with epilepsy of age 0–4 years. Presence of focal onset seizures in children with different epilepsy syndromes needs to be thoroughly considered in the treatment planning of this population of interest.
Im Rahmen der Versorgung von polytraumatisierten (schwerstverletzten) Patienten ist insbesondere die systemische Inflammation zu beachten. Durch das initiale Trauma (“first hit“) kommt es zu einer systemischen Dysregulation der inflammatorischen Kaskaden, wobei sowohl eine überschießende (SIRS/Sepsis) wie auch unterschießende Reaktion (CARS) zu schweren Komplikationen wie Multiorganversagen bis hin zum Tod führen kann. Die notfallmässige chirurgische Versorgung fügt durch multiple Faktoren wie Weichteilverletzung, Blutverlust und Intubation dem Patienten einen “second hit“ zu, welcher sich auf den “first hit“ aufsummieren und besagte Komplikationen induzieren kann. Aufgrund dessen wurden verschiedene Therapiekonzepte entwickelt wie beispielsweise die “Damage control surgery“, welche durch minimalinvasive Techniken die notfallmässig versorgungsbedürftigen Verletzungen temporär stabilisiert/versorgt, bis der Patient sich physiologisch stabilisiert und definitiv versorgt werden kann. Eine weitere Strategie stellt die „Safe Definitive Surgery“ dar, welche eine Synopsis bildet aus zu einen frühzeitiger definitiver Versorgung gepaart mit minimalinvasiven Techniken, um während der Operation multipler Frakturen intraoperativ anhand der Physiologie des Patienten regelmäßig zu reevaluieren und daran zu adjustieren.
Bei der definitiven Versorgung von langen Röhrenknochen im Schaftbereich werden klinisch standardmässig Marknägel verwendet. Hierbei eröffnet man den langen Röhrenkochen am proximalen Eintrittspunkt, bohrt den Knochen intramedullär mittels “Reamer“ auf und führt den Nagel ein, welchen man mittels Schrauben multidimensional in der Corticalis verriegelt. Hierbei stellt die intramedulläre Aufbohrung den kritischsten Schritt dar, da hierbei zum einen Knochenmark austritt und durch den Bohrer Thermonekrosen im Knochen auftreten können sowie auch Knochenpartikel austreten. Um diese Nachteile zu beheben, wurde der “Reamer-Irrigator-Aspirator“ (RIA) entwickelt, welcher nebst der klassischen Bohrfunktion noch eine Spül-Saugfunktion innehat und somit parallel intramedullär eine Kühlung herbeiführt, wie auch das Knochenmark nebst Knochenpartikeln absaugt. Hiervon gibt es eine ältere (RIA 1) und eine neuere (RIA 2) Version, wobei sich diese geringfügig in Grösse des Bohrkopfes und der Saugfunktion wie auch im Handling unterscheiden. Wenig ist jedoch zum aktuellen Zeitpunkt bekannt, welche Auswirkungen diese unterschiedlichen Versionen verglichen mit dem konventionellen “Reamer haben“. Um dies näher zu evaluieren, wurde ein standardisiertes Polytrauma-Modell an 30 Schweinen (Sus scrofa) durchgeführt. Unter konstanter Analgesie wurde nach Erreichen einer standardisierten Baseline an 24 der Tiere ein Polytrauma, bestehend aus unilateraler Femurfraktur, stumpfem Thoraxtrauma inklusive Leberlazeration und hämorrhagischem Schock ausgeübt. Sechs Tiere fungierten als Kontrollgruppe (sham), welche kein Trauma sowie Therapie erhielten, aber sonst gleich behandelt wurden. Die polytraumatisierten Tiere erhielten Therapie nach Schockraum- und ATLS Versorgung nach dem Trauma. Bestehend aus “Abdominal Packing“, Kreislaufstabilisierung und Versorgung der Femurfraktur mittels intramedullärer Nagelung. Die 24 polytraumtisierten Versuchstiere wurden bezüglich der Versorgung der Femurfraktur in drei Gruppen aufgeteilt: 1) Konventionelles Reaming, 2) RIA 1 und 3) RIA 2. An sechs Zeitpunkte (t1 (-1.5h) - t6 (6h)) über 7.5 Stunden erfolgten regelmäßige Blut- wie Urinentnahmen und eine bronchoalveoläre Lavage vor fachgerechtem Exitus am letzen Zeitpunkt. Anschließend wurde mittels ELISA in besagten Proben das Interleukin-6, Interleukin-8, Interleukin-10 und Tumornekrosefaktor-alpha bestimmt und statistische Unterschiede zwischen den Gruppen ermittelt.
Die Ergebnisse legen nahe, dass die Verwendung des Reamer-Irrigator-Aspirator Typ 2 aufgrund spezifischer Modifikationen verglichen mit seinem Vorgänger (RIA Typ 1) eine geringere inflammatorische Immunantwort aufweist. Verglichen mit dem konventionellen Reaming konnte in unserer Versuchsreihe in Hinblick auf entzündliche Mediatoren systemisch wie lokal kein Unterschied zu der Versorgung mittels RIA aufgezeigt werden. Jedoch präsentierte sich bei der Benutzung des konventionellen Reamers auch in unserer Versuchsreihe das Auftreten einer Fett/Lungenembolie, was bereits in der Literatur als eine gängige Komplikation dieses Instrumentariums beschrieben wird. Zusammenfassend ist der Reaming-Irrigator-Aspirator eine modernisierte Version des konventionellem Reamers, welcher multiple Vorteile aufweist, jedoch im Rahmen der Kostensenkung wahrscheinlich erst im weiteren zeitlichen Verlauf regelmäßige Anwendung in der Klinik finden wird.
Adhesion of human pathogenic bacteria to endothelial cells is facilitated by fibronectin interaction

Human pathogenic bacteria circulating in the bloodstream need to find a way to interact with endothelial cells (ECs) lining the blood vessels to infect and colonise the host. The extracellular matrix (ECM) of ECs might represent an attractive initial target for bacterial interaction, as many bacterial adhesins have reported affinities to ECM proteins, in particular to fibronectin (Fn). Here, we analysed the general role of EC-expressed Fn for bacterial adhesion. For this, we evaluated the expression levels of ECM coding genes in different ECs, revealing that Fn is the highest expressed gene and thereby, it is highly abundant in the ECM environment of ECs. The role of Fn as a mediator in bacterial cell-host adhesion was evaluated in adhesion assays of Acinetobacter baumannii, Bartonella henselae, Borrelia burgdorferi, and Staphylococcus aureus to ECs. The assays demonstrated that bacteria colocalised with Fn fibres, as observed by confocal laser scanning microscopy. Fn removal from the ECM environment (FN1 knockout ECs) diminished bacterial adherence to ECs in both static and dynamic adhesion assays to varying extents, as evaluated via absolute quantification using qPCR. Interactions between adhesins and Fn might represent the crucial step for the adhesion of human-pathogenic Gram-negative and Gram-positive bacteria targeting the ECs as a niche of infection.

Abstract
Psychiatric disorders associated with psychological trauma, stress and anxiety are a highly prevalent and increasing cause of morbidity worldwide. Current therapeutic approaches, including medication, are effective in alleviating symptoms of anxiety disorders and posttraumatic stress disorder (PTSD), at least in some individuals, but have unwanted side-effects and do not resolve underlying pathophysiology. After a period of stagnation, there is renewed enthusiasm from public, academic and commercial parties in designing and developing drug treatments for these disorders. Here, we aim to provide a snapshot of the current state of this field that is written for neuropharmacologists, but also practicing clinicians and the interested lay-reader. After introducing currently available drug treatments, we summarize recent/ongoing clinical assessment of novel medicines for anxiety and PTSD, grouped according to primary neurochemical targets and their potential to produce acute and/or enduring therapeutic effects. The evaluation of putative treatments targeting monoamine (including psychedelics), GABA, glutamate, cannabinoid, cholinergic and neuropeptide systems, amongst others, are discussed. We emphasize the importance of designing and clinically assessing new medications based on a firm understanding of the underlying neurobiology stemming from the rapid advances being made in neuroscience. This includes harnessing neuroplasticity to bring about lasting beneficial changes in the brain rather than – as many current medications do – produce a transient attenuation of symptoms, as exemplified by combining psychotropic/cognitive enhancing drugs with psychotherapeutic approaches. We conclude by noting some of the other emerging trends in this promising new phase of drug development.
Long non-coding RNAs (lncRNAs) orchestrate various biological processes and regulate the development of cardiovascular diseases. Their potential therapeutic benefit to tackle disease progression has recently been extensively explored. Our study investigates the role of lncRNA Nudix Hydrolase 6 (NUDT6) and its antisense target fibroblast growth factor 2 (FGF2) in two vascular pathologies: abdominal aortic aneurysms (AAA) and carotid artery disease. Using tissue samples from both diseases, we detected a substantial increase of NUDT6, whereas FGF2 was downregulated. Targeting Nudt6 in vivo with antisense oligonucleotides in three murine and one porcine animal model of carotid artery disease and AAA limited disease progression. Restoration of FGF2 upon Nudt6 knockdown improved vessel wall morphology and fibrous cap stability. Overexpression of NUDT6 in vitro impaired smooth muscle cell (SMC) migration, while limiting their proliferation and augmenting apoptosis. By employing RNA pulldown followed by mass spectrometry as well as RNA immunoprecipitation, we identified Cysteine and Glycine Rich Protein 1 (CSRP1) as another direct NUDT6 interaction partner, regulating cell motility and SMC differentiation. Overall, the present study identifies NUDT6 as a well-conserved antisense transcript of FGF2. NUDT6 silencing triggers SMC survival and migration and could serve as a novel RNA-based therapeutic strategy in vascular diseases.
An abdominal aortic aneurysm (AAA) is a pathological widening of the aortic wall characterized by loss of smooth muscle cells (SMCs), extracellular matrix degradation, and local inflammation. This condition is often asymptomatic until rupture occurs, leading to high morbidity and mortality rates. Diagnosis is mostly accidental and the only currently available treatment option remains surgical intervention. Circular RNAs (circRNAs) represent a novel class of regulatory non-coding RNAs that originate from backsplicing. Their highly stable loop structure, combined with a remarkable enrichment in body fluids, make circRNAs promising disease biomarkers. We investigated the contribution of circRNAs to AAA pathogenesis and their potential application to improve AAA diagnostics. Gene expression analysis revealed the presence of deregulated circular transcripts stemming from AAA-relevant gene loci. Among these, the circRNA to the Ataxia Telangiectasia Mutated gene (cATM) was upregulated in human AAA specimens, in AAA-derived SMCs, and serum samples collected from aneurysm patients. In primary aortic SMCs, cATM increased upon angiotensin II and doxorubicin stimulation, while its silencing triggered apoptosis. Higher cATM levels made AAA-derived SMCs less vulnerable to oxidative stress, compared with control SMCs. These data suggest that cATM contributes to elicit an adaptive oxidative-stress response in SMCs and provides a reliable AAA disease signature.
Introduction: The optimal treatment of patients with spinal infections remains a controversial topic. Within Europe, fundamentally different therapeutic concepts are found. Therefore, the aim of this study was to compare the outcome of patients who received surgical vs. antibiotic treatment alone for primary pyogenic spondylodiscitis in an international cohort analysis.
Materials and Methods: The retrospectively compiled databases of tertiary high-volume spine centers served as the baseline for this study. All documented cases of primary spondylodiscitis treated surgically and conservatively in the period of 2017-2022 were included and grouped according to the therapeutic concept: conservative vs. surgical treatment. Independent investigators collected the relevant clinical and radiological data. The primary endpoint of this study was mortality rate; secondary endpoints were relapse rate and persisting neurological deficit.
Results: A total of 392 patients were included in the analysis (155 females with a mean age of 68 years). Of these, 95 cases were treated conservatively (CoT) and 297 cases were treated surgically (SuT). There was no significant difference (p<0.01) related to patient’s disease characteristics: Lumbar was the main location (n=240, CoT 58/ SuT 182, p=0.97) followed by thoracic (n=70, CoT 24/ SuT 46, p=0,03) and cervical (n=47, CoT 7/ SuT 40, p=0.11) region. A multilocular spinal infection was present in 32 patients (CoT 3/ SuT 29, p=0.04). 181 cases (CoT 36/ SuT 145, p=0.06) presented with an epidural abscess. Neurological deficits were recorded in 100 cases (CoT 26/ SuT 74, p=0.63), and septic conditions in 88 cases (CoT 26/ SuT 62, p=0.19). Pre-existing conditions like Diabetes (p=0.57), renal failure (p= 0.97), hepatopathy (p= 0.15), malignoma (p=0.39) or i.v. drug abuse (p=0.93) did also not differ between the groups. The mortality rate of all conservatively treated was 24.2% (23 cases) and 6.7% (20 cases) in all surgically treated patients (p<0.001). A follow-up of ≥ 6 weeks was available in 289 cases (CoT 83, SuT 206 ). In this subset of patients relapse of infection occurred in six (7.2%) and 23 (11.2%) cases in the conservative and early surgical treatment group, respectively (p=0.69). Persisting neurological deficit was recorded in 21 (25.3%) of conservatively treated and 51 (24.8%) of surgically treated cases (p=0.92).
Conclusion: Whereas relapse rates and persisting neurological deficit were not found to differ significantly, the results of this international data analyses, with their respective limitations, clearly support the growing evidence of a significantly reduced mortality rate after surgical therapy for primary pyogenic spondylodiscitis when compared to conservative treatment regimen.

Although exercise guidelines now recommend exercise for patients with MCI, the long-term effects of exercise in patients with MCI has not been reviewed systematically. The aim was to assess (1) the effectiveness of exercise and physical activity (EXPA) interventions in improving long-term patient-relevant cognitive and non-cognitive outcomes in people with mild cognitive impairment, (2) how well the included trials reported details of the intervention, and (3) the extent to which reported endpoints were in line with patient preferences that were assessed in patient workshops. Following PRISMA guidelines, we performed a systematic review and meta-analysis including randomized controlled trials. A total of ten studies were included after searching in six electronic sources from 1995 onwards. There is a trend that 6 + -month EXPA interventions improve global cognition 12 months after initiation. Evidence on long-term effects of EXPA interventions on non-cognitive health outcomes could not be meaningfully pooled and the individual studies reported mixed results. Workshop participants considered freedom from pain and stress, mood, motivation and self-efficacy to be important, but these outcomes were rarely addressed. Too little information is available on intervention details for EXPA programs to be replicated and confidently recommended for patients with MCI. Cortical tracking of stimulus features (such as the envelope) is a crucial tractable neural mechanism, allowing us to investigate how we process continuous music. We here tested whether cortical and behavioural tracking of beat, typically related to rhythm processing, are modulated by pitch predictability. In two experiments (n=20, n=52), participants’ ability to tap along to the beat of musical sequences was measured for tonal (high pitch predictability) and atonal (low pitch predictability) music. In Experiment 1, we additionally measured participants’ EEG and analysed cortical tracking of the acoustic envelope and of pitch surprisal (using IDyOM). In both experiments, finger-tapping performance was better in the tonal than the atonal condition, indicating a positive effect of pitch predictability on behavioural rhythm processing. Neural data revealed that the acoustic envelope was tracked stronger while listening to atonal than tonal music, potentially reflecting listeners’ violated pitch expectations. Our findings show that cortical envelope tracking, beyond reflecting musical rhythm processing, is modulated by pitch predictability (as well as musical expertise and enjoyment). Stronger cortical surprisal tracking was linked to overall worse envelope tracking, and worse finger-tapping performance for atonal music. Specifically, the low pitch predictability in atonal music seems to draw attentional resources resulting in a reduced ability to follow the rhythm behaviourally. Overall, cortical envelope and surprisal tracking were differentially related to behaviour in tonal and atonal music, likely reflecting differential processing under conditions of high and low predictability. Taken together, our results show diverse effects of pitch predictability on musical rhythm processing.
Non-alcoholic steatohepatitis (NASH) and alcoholic steatohepatitis (ASH) are the leading causes of liver disease worldwide. To identify disease-specific pathomechanisms, we analyzed the lipidome, metabolome and immune cell recruitment in livers in both diseases. Mice harboring ASH or NASH had comparable disease severities regarding mortality rate, neurological behavior, expression of fibrosis marker and albumin levels. Lipid droplet size was higher in NASH than ASH and qualitative differences in the lipidome were mainly based on incorporation of diet-specific fatty acids into triglycerides, phosphatidylcholines and lysophosphatidylcholines. Metabolomic analysis showed downregulated nucleoside levels in both models. Here, the corresponding uremic metabolites were only upregulated in NASH suggesting stronger cellular senescence, which was supported by lower antioxidant levels in NASH as compared to ASH. While altered urea cycle metabolites suggest increased nitric oxide synthesis in both models, in ASH, this depended on increased L-homoarginine levels indicating a cardiovascular response mechanism. Interestingly, only in NASH were the levels of tryptophan and its anti-inflammatory metabolite kynurenine upregulated. Fittingly, high-content immunohistochemistry showed a decreased macrophage recruitment and an increased polarization towards M2-like macrophages in NASH. In conclusion, with comparable disease severity in both models, higher lipid storage, oxidative stress and tryptophan/kynurenine levels were seen in NASH, leading to distinct immune responses.
Lebensqualität, kognitive Leistung und multisensorische Integrationsleistung bei NMOSD Patienten

Die Neuromyelitis-optica-Spektrum-Erkrankung (NMOSD) ist eine entzündliche Autoimmunerkrankung des zentralen Nervensystems (ZNS), die schubweise auftritt und meist in den Anfängen aufgrund der symptomatischen Ähnlichkeit mit der Multiplen Sklerose (MS) verwechselt wird. Primär manifestiert sich die NMOSD in Form von Sehstörungen und sensomotorische Lähmungserscheinungen. Im Krankheitsverlauf treten aber auch bei einem Großteil der Patienten kognitive Defizite auf, wobei vorwiegend das Gedächtnis, die Informationsverarbeitung und die Aufmerksamkeit betroffen sind, die nicht in Routineuntersuchungen erfasst werden. Kognitive Beeinträchtigungen wurden bereits bei der MS beschrieben. Ebenso spielt eine verminderte Lebensqualität bei beiden Erkrankungen eine große Rolle. Analog zu Untersuchungen bei MS Patienten, die gezeigt haben, dass kognitive Beeinträchtigungen mitunter ursächlich für die niedrige Lebensqualität sind, wird in dieser Arbeit postuliert, dass auch bei NMOSD Patienten das Ausmaß an kognitiven Dysfunktionen mit dem Grad an Einbußen in der Lebensqualität zusammenhängt. Ferner sollen weitere Prädiktoren ermittelt werden, welche einen Einfluss auf die Lebensqualität haben, wie bereits bestätigt körperliche Einschränkungen. Es wird erwartet, dass NMOSD Patienten von einer verminderten Lebensqualität berichten, die von den schlechteren Ergebnissen in den neuropsychologischen Tests vorhergesagt werden kann.
Zur Untersuchung der Kognition wurde in der vorliegenden Arbeit neben etablierten neuropsychologischen Tests auch die multisensorische Integrationsleistung mithilfe des SiFI Paradigmas angewandt, welche bereits bei MS Patienten und Patienten mit leichten kognitiven Defiziten (mild cognitive impairment; MCI) auffällige Daten lieferte und für eine Testung der globalen Kognitionsleistung genutzt werden konnte. Der Grund für den Einsatz der SiFI waren die nachgewiesenen Hirnkorrelate bei multisensorischer Integration, welche ebenfalls bereits bei kognitiver Dysfunktion festgestellt wurden, wie Atrophien, Konnektivitätsstörungen und Auffälligkeiten in der Transmission bestimmter Neurotransmitter. Ziel dieser Anwendung ist eine Implementierung der SiFI in den Klinikalltag zur erleichterten Erfassung kognitiver Defizite. Viele bekannte neuropsychologischen Tests sind entweder zu teuer, zu lang, abhängig von der sprachlichen Fähigkeit oder für die Patienten zu anstrengend. Die SiFI wäre daher eine gute Alternative als Marker kognitiver Defizite.
20 NMOSD Patienten wurden zu ihrer Lebensqualität (EQ-5D) sowie ihrem psychopathologischen Zustand (SCL-90-R) befragt und es wurde eine umfassende neuropsychologische Testung durchgeführt. Zur Diagnostik der multisensorischen Integrationsleistung wurde die SiFI Aufgabe herangezogen. Die Ergebnisse deuten auf eine verminderte kognitive Leistung mit mittelhohen Werten in den Fragebögen zur Lebensqualität. NMOSD Patienten nahmen die Illusion in der SiFI Aufgabe bei längeren Intervallen wahr, vergleichbar mit MS und MCI Patienten. Dies deutet auf eine verzögerte Integration sensorischer Informationen.
Angefangen mit einem Einblick über die Erkrankung und Darstellung des bisherigen Wissenschaftsstands zu den einzelnen Konstrukten und ihrer Zusammenhänge wird das Studiendesign vorgestellt und die Ergebnisse angegeben und interpretiert. Abschließend folgen eine kritische Beurteilung und Zusammenfassung der vorliegenden Daten mit Ausblick auf weitere Forschungsziele.
The interaction of Eph receptor tyrosine kinases with their transmembrane ligands; the ephrins, is important for the regulation of cell-cell communication. Ephrin-Eph signaling is probably best known for the discrimination of arterial and venous territories by repulsion of venous endothelial cells away from those with an arterial fate. Ultimately, cell repulsion is mediated by initiating the collapse of the actin cytoskeleton in membrane protrusions. Here, we investigated the role of the Ena/VASP family of actin binding proteins in endothelial cell repulsion initiated by ephrin ligands. Human endothelial cells dynamically extended sheet-like lamellipodia over ephrin-B2 coated surfaces. While lamellipodia of control siRNA transfected cells rapidly collapsed, resulting in a pronounced cell repulsion from the ephrin-B2 surfaces, the knockdown of Ena/VASP proteins impaired the cytoskeletal collapse of membrane protrusions and the cells no longer avoided the repulsive surfaces. Mechanistically, ephrin-B2 stimulation elicited the EphB-mediated tyrosine phosphorylation of VASP, which abrogated its interaction with the focal adhesion protein Zyxin. Nck2 was identified as a novel VASP binding protein, which only interacted with the tyrosine phosphorylated VASP protein. Nck links Eph-receptors to the actin cytoskeleton. Therefore, we hypothesize that Nck-Ena/VASP complex formation is required for actin reorganization and/or Eph receptor internalization downstream of ephrin-Eph interaction in endothelial cells, with implications for endothelial navigation and pathfinding.
Hintergrund: Der Rettungsdienst versorgt täglich viele Patient/-innen in unterschiedlichen Umgebungen und ist damit auch potentieller Überträger nosokomialer Infektionen. Zur Händehygiene, als entscheidende Säule der Infektionsprophylaxe, liegen bislang nur wenige Daten aus dem Rettungsdienst vor.
Methoden: Prospektive multizentrische Studie mit Fragebogen zur Selbst- und Fremdeinschätzung der Compliance und beeinflussender Faktoren (abgeleitet von der WHO Perception Survey for Health-Care Workers) sowie direkte Compliance-Beobachtung nach WHO-Standard bei Rettungsdienstpersonal zweier Berufsfeuerwehren in Deutschland.
Ergebnisse: Es wurden 207 Fragebögen eingereicht und während ca. 66h Beobachtungszeit wurden 674 Händedesinfektionsgelegenheiten protokolliert. Der präventive Effekt der HH wurde allgemein von den Mitarbeitenden anerkannt. Die Selbsteinschätzung (MW: 80%) und beobachtete Compliance-Rate (38%) zeigten eine deutliche Diskrepanz und die Compliance variierte zwischen den verschiedenen Indikationen. Besonders niedrig zeigte sich die Compliance rund um die Durchführung aseptischer Tätigkeiten. Hier zeichnete sich ein geringes Risikobewusstsein für nicht sichtbare Verunreinigungen ab. Hürden für die Umsetzung der Händehygiene stellten vor allem die Vorrangigkeit anderer Maßnahmen, Unterbrechung des Arbeitsablaufes und Zeitmangel dar.
Schlussfolgerungen: Die beobachtete Compliance-Rate im Rettungsdienst lag unterhalb der innerklinischen Durchschnittswerte. Insbesondere die Compliance im Rahmen aseptischer Tätigkeiten muss dringend gesteigert werden. Dies erfordert einen multimodalen Lösungsansatz, der die Optimierung der Ausbildung, Algorithmen, Materialverfügbarkeit und Praktikabilität der Händedesinfektion im Rettungsdienst beinhaltet.
This research article examines the dual impact of protests on COVID-19 spread, a challenge for policymakers balancing public health and the right to assemble. Using a game theoretical model, it shows that protests can shift infection risks between counties, creating a dilemma for regulators. The empirical study analyzes two German protests in November 2020 using proprietary data from a bus-shuttle service, finding evidence to support the assumption that protests can shift infection risks. The article concludes by discussing the implications of these findings for policymakers, highlighting that regulators’ individually rational strategic decisions may lead to inefficient outcomes.
Memory consolidation tends to be less robust in childhood than adulthood. However, little is known about the corresponding functional differences in the developing brain that may underlie age-related differences in retention of memories over time. This study examined system-level memory consolidation of object-scene associations after learning (immediate delay), one night of sleep (short delay), as well as two weeks (long delay) in 5-to-7-year-old children (n = 49) and in young adults (n = 39), as a reference group with mature consolidation systems. Particularly, we characterized how functional neural activation and reinstatement of neural patterns change over time, assessed by functional magnetic resonance imaging combined with representational (dis)similarity analysis (RSA). Our results showed that memory consolidation in children was less robust (i.e., more forgetting) compared to young adults. For correctly retained remote memories, young adults showed increased neural activation from short to long delay in neocortical (parietal, prefrontal and occipital) and cerebellar brain regions, while children showed increased neural activation in prefrontal and decrease in neural activity in parietal brain regions over time. In addition, there was an overall attenuated scene-specific memory reinstatement of neural patterns in children compared to young adults. At the same time, we observed category-based reinstatement in medial-temporal, neocortical (prefrontal and parietal), and cerebellar brain regions only in children. Taken together, 5-to-7-year-old children, compared to young adults, show less robust memory consolidation, possibly due to difficulties in engaging in differentiated neural reinstatement in neocortical mnemonic regions during retrieval of remote memories, coupled with relying more on gist-like, category-based neural reinstatement.
Human functional brain connectivity can be temporally decomposed into states of high and low cofluctuation, defined as coactivation of brain regions over time. Rare states of particularly high cofluctuation have been shown to reflect fundamentals of intrinsic functional network architecture and to be highly subject-specific. However, it is unclear whether such network-defining states also contribute to individual variations in cognitive abilities – which strongly rely on the interactions among distributed brain regions. By introducing CMEP, a new eigenvector-based prediction framework, we show that as few as 16 temporally separated time frames (< 1.5% of 10min resting-state fMRI) can significantly predict individual differences in intelligence (N = 263, p < .001). Against previous expectations, individual’s network-defining time frames of particularly high cofluctuation do not predict intelligence. Multiple functional brain networks contribute to the prediction, and all results replicate in an independent sample (N = 831). Our results suggest that although fundamentals of person-specific functional connectomes can be derived from few time frames of highest connectivity, temporally distributed information is necessary to extract information about cognitive abilities. This information is not restricted to specific connectivity states, like network-defining high-cofluctuation states, but rather reflected across the entire length of the brain connectivity time series.
The traditional view on coding in the cortex is that populations of neurons primarily convey stimulus information through the spike count. However, given the speed of sensory processing, it has been hypothesized that sensory encoding may rely on the spike-timing relationships among neurons. Here, we use a recently developed method based on Optimal Transport Theory called SpikeShip to study the encoding of natural movies by high-dimensional ensembles of neurons in visual cortex. SpikeShip is a generic measure of dissimilarity between spike train patterns based on the relative spike-timing relations among all neurons and with computational complexity similar to the spike count. We compared spike-count and spike-timing codes in up to N > 8000 neurons from six visual areas during natural video presentations. Using SpikeShip, we show that temporal spiking sequences convey substantially more information about natural movies than population spike-count vectors when the neural population size is larger than about 200 neurons. Remarkably, encoding through temporal sequences did not show representational drift both within and between blocks. By contrast, population firing rates showed better coding performance when there were few active neurons. Furthermore, the population firing rate showed memory across frames and formed a continuous trajectory across time. In contrast to temporal spiking sequences, population firing rates exhibited substantial drift across repetitions and between blocks. These findings suggest that spike counts and temporal sequences constitute two different coding schemes with distinct information about natural movies.
Control of cell proliferation is critical for the lymphocyte life cycle. However, little is known on how stage-specific alterations in cell-cycle behavior drive proliferation dynamics during T-cell development. Here, we employed in vivo dual-nucleoside pulse labeling combined with determination of DNA replication over time as well as fluorescent ubiquitination-based cell-cycle indicator mice to establish a quantitative high-resolution map of cell-cycle kinetics of thymocytes. We developed an agent-based mathematical model of T-cell developmental dynamics. To generate the capacity for proliferative bursts, cell-cycle acceleration followed a 'stretch model', characterized by simultaneous and proportional contraction of both G1 and S phase. Analysis of cell-cycle phase dynamics during regeneration showed tailored adjustments of cell-cycle phase dynamics. Taken together, our results highlight intrathymic cell-cycle regulation as an adjustable system to maintain physiologic tissue homeostasis and foster our understanding of dysregulation of the T-cell developmental program.
Alzheimer’s Disease (AD) is a progressive and irreversible neurodegenerative disorder, characterized by the accumulation of abeta-amyloid aggregates, which triggers tau hyperphosphorylation and neuronal loss. While the precise mechanisms underlying neurodegeneration in AD are not entirely understood, it is known that loss of proteostasis is implicated in this process. Maintaining neuronal proteostasis requires proper transfer RNA (tRNA) modifications, which are crucial for optimal translation. However, research into tRNA epitranscriptome in AD is limited, and it is not yet clear how alterations in tRNA modifying enzymes and tRNA modifications might contribute to disease progression. Here, we report that expression of the tRNA modifying enzyme ELP3 is reduced in the brain of AD patients and amyloid AD mouse models, suggesting ELP3 is implicated in proteostasis dysregulation observed in AD. To investigate the role of ELP3 specifically in neuronal proteostasis impairments in the context of amyloid pathology, we analyzed SH-SY5Y neuronal cells carrying the amyloidogenic Swedish familial AD mutation in the APP gene (SH-SWE) or the wild-type gene (SH-WT). Similarly to the amyloid mouse models, SH-SWE exhibited reduced levels of ELP3 which was associated with tRNA hypomodifications and reduced abundance, as well as proteostasis impairments. Furthermore, the knock-down of ELP3 in SH-WT recapitulated the proteostasis impairments observed in SH-SWE cells. Importantly, the correction of tRNA deficits due to ELP3 reduction rescued and reverted proteostasis impairments of SH-SWE and SH-WT knock-down for ELP3, respectively. Additionally, SH-WT exposed to the secretome of SH-SWE or synthetic amyloid aggregates recapitulate the SH-SWE phenotype, characterized by reduced ELP3 expression, tRNA hypomodification and increased protein aggregation. Taken together, our data suggest that amyloid pathology dysregulates neuronal proteostasis through the reduction of ELP3 and tRNA modifications. This study highlights the modulation of tRNA modifications as a potential therapeutic avenue to restore neuronal proteostasis in AD and preserve neuronal function.
Endothelial tip cells are essential for VEGF-induced angiogenesis, but underlying mechanisms are elusive. Endothelial-specific deletion of EVL, a member of the mammalian Ena/VASP protein family, reduced the expression of the tip cell marker protein endothelial cell specific molecule-1 (Esm1) and compromised the radial sprouting of the vascular plexus in the postnatal mouse retina. The latter effects could at least partly be attributed to reduced VEGF receptor 2 (VEGFR2) internalization and signaling but the underlying mechanisms(s) are not fully understood. In the present study, we revealed that the expression of the long non-coding RNA H19 was significantly reduced in endothelial cells from postnatal EVL-/- mice and in siRNA-transfected human endothelial cells under hypoxic conditions. H19 was recently shown to promote VEGF expression and bioavailability via Esm1 and hypoxia inducible factor 1α (HIF-1α). Similar to EVL-/- mice, the radial outgrowth of the vascular plexus was significantly delayed in the postnatal retina of H19-/- mice. In summary, our data suggests that loss of EVL not only impairs VEGFR2 internalition and downstream signaling, but also impairs VEGF expression and bioavailability in the hypoxic retina via downregulation of lncRNA H19.
To understand the neural mechanisms underlying brain function, neuroscientists aim to quantify causal interactions between neurons, for instance by perturbing the activity of neuron A and measuring the effect on neuron B. Recently, manipulating neuron activity using light-sensitive opsins, optogenetics, has increased the specificity of neural perturbation. However, using widefield optogenetic interventions, multiple neurons are usually perturbed, producing a confound -- any of the stimulated neurons can have affected the postsynaptic neuron making it challenging to discern which neurons produced the causal effect. Here, we show how such confounds produce large biases in interpretations. We explain how confounding can be reduced by combining instrumental variables (IV) and difference in differences (DiD) techniques from econometrics. Combined, these methods can estimate (causal) effective connectivity by exploiting the weak, approximately random signal resulting from the interaction between stimulation and the absolute refractory period of the neuron. In simulated neural networks, we find that estimates using ideas from IV and DiD outperform naive techniques suggesting that methods from causal inference can be useful to disentangle neural interactions in the brain.
Background Vasoplegic syndrome is frequently observed during cardiac surgery and resembles a complication of high mortality and morbidity. There is a clinical need for therapy and prevention of vasoplegic syndrome during complex cardiac surgical procedures. Therefore, we investigated different strategies in a porcine model of vasoplegia.
Methods We evaluated new medical therapies and prophylaxis to avoid vasoplegic syndrome in a porcine model. After induction of anesthesia, cardiopulmonary bypass was established through median sternotomy and central cannulation. Prolonged aortic cross-clamping (120 min) simulated a complex surgical procedure. The influence of sevoflurane-guided anesthesia (sevoflurane group) and the administration of glibenclamide (glibenclamide group) were compared to a control group, which received standard anesthesia using propofol. Online hemodynamic assessment was performed using PiCCO® measurements. In addition, blood and tissue samples were taken to evaluate hemodynamic effects and the degree of inflammatory response.
Results Glibenclamide was able to break through early vasoplegic syndrome by raising the blood pressure and systemic vascular resistance as well as less need of norepinephrine doses. Sevoflurane reduced the occurrence of the vasoplegic syndrome in the mean of stable blood pressure and less need of norepinephrine doses.
Conclusion Glibenclamide could serve as a potent drug to reduce effects of vasoplegic syndrome. Sevoflurane anesthesia during cardiopulmonary bypass shows less occurrence of vasoplegic syndrome and therefore could be used to prevent it in high-risk patients.

The Asian tiger mosquito Aedes albopictus is currently spreading across Europe, facilitated by climate change and global transportation. It is a vector of arboviruses causing human diseases such as chikungunya, dengue hemorrhagic fever and Zika fever. For the majority of these diseases, no vaccines or therapeutics are available. Options for the control of Ae. albopictus are limited by European regulations introduced to protect biodiversity by restricting or phasing out the use of pesticides, genetically modified organisms (GMOs) or products of genome editing. Alternative solutions are thus urgently needed to avoid a future scenario in which Europe faces a choice between prioritizing human health or biodiversity when it comes to Aedes-vectored pathogens. To ensure regulatory compliance and public acceptance, these solutions should preferably not be based on chemicals or GMOs and must be cost-efficient and specific. The present review aims to synthesize available evidence on RNAi-based mosquito vector control and its potential for application in the European Union. The recent literature has identified some potential target sites in Ae. albopictus and formulations for delivery. However, we found little information concerning non-target effects on the environment or human health, on social aspects, regulatory frameworks, or on management perspectives. We propose optimal designs for RNAi-based vector control tools against Ae. albopictus (target product profiles), discuss their efficacy and reflect on potential risks to environmental health and the importance of societal aspects. The roadmap from design to application will provide readers with a comprehensive perspective on the application of emerging RNAi-based vector control tools for the suppression of Ae. albopictus populations with special focus on Europe.
Bei über der Hälfte der Patienten mit einer operationsbedürftigen MI liegt gleichzeitig eine Insuffizienz der TK vor. In den meisten Fällen handelt es sich hierbei um eine funktionelle Insuffizienz welche aus einer RV-Pathologie, bedingt durch eine Volumen- oder Druckbelastung resultiert. In der Vergangenheit bestand die Überzeugung eine konservative Behandlung der TI sei ausreichend und nach Beheben der ursächlichen Grunderkrankung sei diese selbstlimitierend. Neuere Studien konnten jedoch belegen, dass eine bestehende TI auch nach operativer Versorgung einer ursächlichen LV-pathologie keine ausreichende Rückbildungstendenz aufweist, sondern im Verlauf sogar noch progredient ist. Die persistierende TI führt zu einer deutlich erhöhten Morbidität und Mortalität. Ist eine zweizeitige Operation an der TK im Verlauf erforderlich, so ist die Früh- und Spätmortalität deutlich erhöht und die Langzeitergebnisse sind schlecht. Diese Ergebnisse haben dazu geführt, dass die Indikation zur operativen Versorgung der TI ≥ 2 zu einem früheren Zeitpunkt und weniger restriktiv gestellt wird. Jedoch weisen die aktuellen ESC/EACTS-Guidelines insbesondere hinsichtlich der Versorgung einer TI < 2 nur einen niedrigen Evidenz-Grad auf, der richtige Operationszeitpunkt bleibt weiterhin umstritten. Der Grund für eine eher zurückhaltende Einstellung ist vor allem die Angst vor einer erhöhten Morbidität und Mortalität durch den additiven Eingriff. Zudem sind kaum Studien zu einer operativen Versorgung einer TI < 2 vorhanden. Ziel der vorliegenden Arbeit war daher der Gewinn weiterer Erkenntnisse, dieses bis dato kaum untersuchten Patientengutes, insbesondere hinsichtlich der peri- und postoperativen Mortalität, sowie der Auswirkungen der prophylaktischen TKR im Langzeitverlauf. Im Rahmen dieser monozentrischen, prospektiven Studie wurden 264 Patienten eingeschlossen, welche sich im Zeitraum von 2009 bis 2015 einer TKR im Rahmen einer MKR an der Klinik für Thorax- Herz- und thorakaler Gefäßchirurgie des Universitätsklinikums Frankfurt am Main unterzogen. Das Patientenkollektiv wurde nachfolgend in zwei Gruppen unterteilt, nach dem Schweregrad der präoperativ bestehenden TI in eine „prophylaktische” pTKR-Gruppe bei einer TI < 2 und eine „therapeutische” tTKR-Gruppe bei einer TI ≥ 2. Primärer Endpunkt war die Erfassung der Früh- und Spätmortalität. Sekundäres Endziel war die Untersuchung des postoperativen Verlaufes der TI, sowie der Verlauf der kardialen Funktion. Die 30-Tages-Mortalität betrug 12% in der pTKR-Gruppe und 17% in der tTKR-Gruppe. Der wichtigste Einflussfaktor war die EKZ-Dauer. Der Unterschied zwischen den beiden Gruppen war statistisch nicht signifikant. Im Vergleich zu anderen Studien zeigte sich eine höhere 30-Tages-Mortalität, jedoch handelte es sich bei diesen zumeist um einen isolierten Eingriff an der Mitralklappe, mit einer deutlich kürzeren EKZ. Hinsichtlich der Gesamtmortalität zeigte sich ein 1-, 5- und 7-Jahres-Überleben in der pTKR-Gruppe von 81%, 66% und 56%, sowie 65%, 52% und 41% in der tTKR-Gruppe. Die höhere Gesamtmortalität dieser Studie im Vergleich zu anderen Arbeiten ist durch das deutlich ältere und multimorbide Patientenkollektiv erklärt. Bei dem Vergleich von Patienten, welche im Rahmen dieser Studie eine prophylaktische TKR erhielten gegenüber den Patienten, bei welchen im Rahmen von Vergleichsarbeiten bewusst auf eine TKR verzichtet wurde (NTKR-Gruppe), zeigte sich ein verbessertes Langzeitüberleben der pTKR-Gruppe. Bei den Arbeiten, bei welchen kein Überlebensvorteil unserer pTKR-Gruppe, gegenüber deren NTKR-Gruppe gezeigt werden konnte, zeigten sich dennoch positive Effekte der begleitenden Klappenoperation. Die erhöhte Frühsterblichkeit dieser Arbeit ist dem Umstand geschuldet, dass durch die zur Indikationsstellung herangezogenen Risikofaktoren ein Hochrisikokollektiv selektioniert wurde, mit einer konsekutiv erhöhten Sterblichkeit auch abhängig vom Ausmass der TI. Bereits in der Vergangenheit konnte eine Verbesserung des Langzeitüberlebens durch die Durchführung einer TKR bei einer TI ≥ 2, gegenüber dem Verzicht auf diese nachgewiesen werden. Anhand des Vergleiches von anderen Arbeiten mit der vorliegenden Arbeit konnte dieser Überlebensvorteil auch für die Durchführung einer pTKR bei einer TI < 2 gezeigt werden. Zudem konnte dargelegt werden, dass es sich bei der begleitenden pTKR um eine effektive und dauerhafte Methode zur Vermeidung einer Klappeninsuffizienz handelt. Somit konnte gezeigt werden, dass auch bei Patienten mit einer TI < 2 eine additiven TKR die Entwicklung einer späten TI verhindert.
Knowledge is limited as to how prior SARS-CoV-2 infection influences cellular and humoral immunity after booster-vaccination with bivalent BA.4/5-adapted mRNA-vaccines, and whether vaccine-induced immunity correlates with subsequent infection. In this observational study, individuals with prior infection (n=64) showed higher vaccine-induced anti-spike IgG antibodies and neutralizing titers, but the relative increase was significantly higher in non-infected individuals (n=63). In general, both groups showed higher neutralizing activity towards the parental strain than towards Omicron subvariants BA.1, BA.2 and BA.5. In contrast, CD4 or CD8 T-cell levels towards spike from the parental strain and the Omicron subvariants, and cytokine expression profiles were similar irrespective of prior infection. Breakthrough infections occurred more frequently among previously non-infected individuals, who had significantly lower vaccine-induced spike-specific neutralizing activity and CD4 T-cell levels. Thus, the magnitude of vaccine-induced neutralizing activity and specific CD4 T-cells after bivalent vaccination may serve as a correlate for protection in previously non-infected individuals.
Untersuchung der Expression von Wachstumsfaktoren in reseziertem Hirngewebe von Epilepsiepatienten

Hintergrund: Die Epilepsie gehört zu den häufigsten chronischen neurologischen Erkrankungen beim Menschen. Bei Patienten mit mesialer TLE und Hippocampussklerose besteht die höchste Wahrscheinlichkeit, eine medikamentöse Therapierefraktärität zu entwickeln. Die Ursache der Hippocampussklerose sowie die ursächlichen Mechanismen sind nicht bekannt. Allerdings kann eine initiale Schädigung, wie etwa komplizierte Fieberkrämpfe im Kindesalter, Schädel-Hirn-Traumata, Schlaganfälle, entzündliche Prozesse oder Ähnliches, für die Entwicklung einer Hippocampussklerose prädisponieren. Diese kann anschließend nach einer klinisch stummen Latenzperiode zur Entwicklung spontaner epileptischer Anfälle und der Diagnose einer Epilepsie führen. Im Rahmen der Epileptogenese, also der Entstehung und Progression der Epilepsie kommt es zu Wachstumsprozessen, weshalb eine Beteiligung von neurotrophen Wachstumsfaktoren naheliegend war. Das Ziel dieser Arbeit war die vergleichende Untersuchung resezierter Hippocampi auf Wachstumsfaktoren, um semiquantitative Daten zu deren Verteilung bei Epilepsiepatienten zu erhalten. Des Weiteren war die Korrelation mit den klinischen Daten der Patienten von besonderem Interesse, da so Hinweise auf mögliche Zusammenhänge zwischen dem klinischen Erscheinungsbild und der Expression der Wachstumsfaktoren gewonnen werden konnten.
Methoden: Bei dem in der vorliegenden Arbeit untersuchten Gewebe handelt es sich um Hippocampi von 21 Patienten mit TLE, die epilepsiechirurgisch therapiert wurden. Die Schnitte der paraffinierten Hippocampi wurden mittels Immunhistochemie auf die Wachstumsfaktoren BDNF, FGF2, GDNF, GMFB und PDGF-B untersucht. Im Anschluss wurden die Schnitte gescannt und die Zellen mittels eines Algorithmus identifiziert und ausgewertet. Diese experimentellen Daten wurden anschließend mit den klinischen Daten der Patienten korreliert.
Ergebnisse: Es fand sich eine signifikante Korrelation zwischen der Expression von GMFB und dem postoperativen Outcome der Patienten. Des Weiteren fanden sich auch Korrelationen zwischen der präoperativen Anfallsfrequenz und der Expression von BDNF sowie GDNF. Auch die Epilepsiedauer korrelierte mit der Expression von BDNF. Zudem fanden sich Korrelationen zwischen den Ergebnissen der neuropsychologischen Testungen und der Expression von BDNF, sowie PDGF-B.
Diskussion: Die vorliegende Arbeit liefert einige Daten, die Hinweise für nachfolgende Untersuchungen geben können. Sowohl für die Anfallsfrequenz, als auch für die Epilepsiedauer fanden sich signifikante Korrelationen mit BDNF. Beides ist passend zu den vermuteten und zum Teil in der Literatur beschriebenen Mechanismen im Rahmen der Epilepsie, also einer postiktalen Hochregulation von Wachstumsfaktoren beziehungswiese des Zugrundegehens von Zellen im Verlauf der Erkrankung und damit zu einer reduzierten Expression von Wachstumsfaktoren. Geschlechterabhängige Unterschiede in der Expression der Wachstumsfaktoren fanden sich, passend zu der vorhandenen Literatur, nicht. Interessant ist, dass sowohl Geschlechtshormone als auch anfallssuppressive Medikamente einen Einfluss auf die Expression der Wachstumsfaktoren haben können.
Bis heute ist kein Biomarker bekannt, der eine Vorhersage über den Erfolg einer operativen Therapie bei therapierefraktären TLE treffen kann. Da meine Daten eine Korrelation von GMFB und dem postoperativen Outcome zeigen, bietet es sich für weitere Untersuchungen an, GMFB als präoperativen Biomarker zu nutzen. zu können, wäre eine einfachere Probengewinnung beispielsweise aus Blut, Liquor, Urin oder Speichel notwendig. Im Sinne einer „Liquid Biopsy“ könnte so der Erfolg einer chirurgischen Therapie weiter objektiviert werden, was die Entscheidungsfindung einfacher und risikoärmer gestalten würde.
Background and purpose: In patients with epilepsies of structural origin, brain atrophy and pathological alterations of the tissue microstructure extending beyond the putative epileptogenic lesion have been reported. However, in patients without any evidence of epileptogenic lesions on diagnostic magnetic resonance imaging (MRI), impairment of the brain microstructure has been scarcely elucidated. Using multiparametric quantitative (q) magnetic resonance imaging MRI, we aimed to investigate diffuse impairment of the microstructural tissue integrity in MRI-negative focal epilepsy patients.
Methods: 27 MRI-negative patients with focal epilepsy (mean age 33.1 ± 14.2 years) and 27 matched healthy control subjects underwent multiparametric qMRI including T1, T2, and PD mapping at 3 T. After tissue segmentation based on synthetic anatomies, mean qMRI parameter values were extracted from the cerebral cortex, the white matter (WM) and the deep gray matter (GM) and compared between patients and control subjects. Apart from calculating mean values for the qMRI parameters across the respective compartments, voxel-wise analyses were performed for each tissue class.
Results: There were no significant differences for mean values of quantitative T1, T2, and PD obtained from the cortex, the WM and the deep GM between the groups. Furthermore, the voxel-wise analyses did not reveal any clusters indicating significant differences between patients and control subjects for the qMRI parameters in the respective compartments.
Conclusions: Based on the employed methodology, no indication for an impairment of the cerebral microstructural tissue integrity in MRI-negative patients with focal epilepsy was found in this study. Further research will be necessary to identify relevant factors and mechanisms contributing to microstructural brain tissue damage in various subgroups of patients with epilepsy.
Die chronische myeloische Leukämie (CML) ist eine klonale myeloproliferative Neoplasie und hat ihren Ursprung in transformierten pluripotenten Stammzellen im Knochenmark. Der Krankheitsentstehung liegt eine reziproke chromosomale Translokation zugrunde, in deren Folge ein neues Fusionsgen, das sogenannte Philadelphia-Chromosom, entsteht. Das hiervon codierte Genprodukt ist eine Tyrosinkinase mit konstitutiver Aktivität mit resultierender unkontrollierter Signaltransduktion. Gegen diese Tyrosinkinase existiert eine molekular zielgerichtete Therapie, die Tyrosinkinase-Inhibitoren (TKI).
Den Therapiestandard stellt die Behandlung mit einem TKI dar. Seit einigen Jahren existiert jedoch das Therapieziel der therapiefreien Remission (TFR), bei dem CML-Patienten mit einem guten molekularen Ansprechen den TKI nach einiger Zeit absetzen und unter engmaschigen Kontrollen therapiefrei bleiben. Ungefähr die Hälfte dieser Patienten erleidet kein molekulares Rezidiv und bleibt langfristig in TFR. Zu der Thematik der TFR existieren zahlreiche klinische Studien, die die Umsetzbarkeit und die Sicherheit eines Absetzversuchs belegen und Kriterien definiert haben, die für einen Absetzversuch erfüllt sein sollten. In dieser Dissertation wird die Umsetzung der TFR im klinischen Alltag onkologischer Praxen untersucht. Es wird untersucht, ob die Studienergebnisse mit den TFR-Raten und den identifizierten Einflussfaktoren auf den Praxisalltag übertragbar sind und ob das Therapieziel TFR in den klinischen Alltag integrierbar ist. Hierfür werden die Daten von 61 CML-Patienten mit einem Absetzversuch aus fünf onkologischen Praxen retrospektiv ausgewertet. Erhoben werden Parameter der Routineversorgung und die Ergebnisse der Kontrolluntersuchungen während der TFR werden dokumentiert und ausgewertet. Die TFR-Raten von ca. 50%, die in den Absetz-Studien beobachtet wurden, finden sich im hier untersuchten Patientenkollektiv wieder. Mithilfe der binären logistischen Regression wird getestet, ob bestimmte Faktoren, wie die Therapiedauer und das verwendete Medikament vor dem Absetzen, einen signifikanten Einfluss auf den Verlauf der TFR haben. Hier kann kein signifikanter Einflussfaktor identifiziert werden. Es lässt sich eine signifikant längere therapiefreie Überlebensdauer bei den Patienten zeigen, die mit einem TKI der zweiten Generation vor dem Absetzen behandelt wurden.
Zusammenfassend lässt sich sagen, dass die TFR bei geeigneten CML-Patienten in der Praxis umsetzbar und sicher ist und fest im klinischen Alltag onkologischer Praxen verankert sein sollte.
In der vorliegenden Arbeit wurden die Proteine Vlp15/16 und GlpQ aus B. miyamotoi hinsichtlich ihrer Eigenschaft, mit Plasminogen zu interagieren, charakterisiert.
Da einige Fälle von ZNS-Beteiligungen bei B. miyamotoi-Infektionen berichtet wurden, ist anzunehmen, dass diese Borrelienspezies über molekulare Mechanismen zur Überwindung der Blut-Hirn-Schranke verfügt. Eine solche Strategie könnte die Bindung wirtseigener Proteasen wie z.B. Plasminogen sein, um Komponenten der extrazellulären Matrix zu degradieren und dadurch die Dissemination des Erregers zu erleichtern.
Während Vmps, zu welchen auch Vlp15/16 gehört, als membranständige Proteine durch Variation der antigenen Oberflächenmatrix zur Immunevasion des Erregers beitragen, ist GlpQ bei der Hydrolyse von Phospholipiden in den Zellstoffwechsel eingebunden. Trotz dieser unterschiedlichen Funktionen, die den beiden Proteinen zukommen, binden beide Moleküle Plasminogen. Die Eigenschaften dieser Interaktion wurden in dieser Arbeit im Detail untersucht. Die Ergebnisse zeigen, dass Vlp15/16 und GlpQ Plasminogen konzentrationsabhängig binden und die Dissoziationskonstanten (Vlp15/16:Kd = 354 nM ± 62 nM; GlpQ: Kd = 413 nM ± 72 nM) für beide Proteine im Bereich der Serumkonzentration von 2 µM liegen. Darüber hinaus konnte gezeigt werden, dass den beiden Proteinen unterschiedliche Mechanismen zugrunde liegen, Plasminogen zu binden. Während die erhobenen Daten für Vlp15/16 darauf hindeuten, dass Lysin-Reste essenziell für die Interaktion sind, scheinen bei GlpQ ionische Wechselwirkungen von Bedeutung zu sein.
Um die Beteiligung von C-terminal lokalisierten Lysin-Resten für die PlasminogenBindung von GlpQ nachzuweisen, wurden Varianten mit einzelnen Lysin-Substitutionen an zwei unterschiedlichen Positionen (333 und 334) sowie eine Variante mit einer Zweifach-Substitution (GlpQ-K333A-K334A) generiert. Die Bindungsanalysen ergaben, dass insbesondere der Lysin-Rest an Position 334 bei der Interaktion mit Plasminogen beteiligt ist.
Die funktionellen Analysen zeigten, dass das an Vlp15/16 beziehungsweise GlpQ gebundene Plasminogen zu Plasmin aktiviert werden konnte und darüber hinaus dazu in der Lage war, das physiologische Substrat Fibrinogen zu degradieren.
Abschließend wurde die Plasminogen-Bindung an nativen B. miyamotoi-Zellen mittels Immunfluoreszenz-Mikroskopie nachgewiesen.
Die Ergebnisse dieser Arbeit weisen Vlp15/16 und GlpQ als Plasminogen-bindende Proteine aus, mit deren Hilfe B. miyamotoi befähigt ist, Komponenten der extrazellulären Matrix zu degradieren und somit prinzipiell zur Dissemination des Erregers beizutragen.
Abdominal aortic aneurysm (AAA) is the most common type of aortic aneurysm, which is defined as a dilation of the abdominal aorta over 3.0 cm or more. Surgical repair is the golden standard for the treatment of AAA, in which open surgical repair (OSR) and endovascular aneurysm repair (EVAR) are the main approaches. Technically speaking, the lesion segment of aueurysm is completely replaced by a graft during OSR, while in EVAR, the lesion is insulated by a stentgraft. EVAR is a less invasive treatment than OSR and shows a lower early mortality rate, although the long-term advantages of EVAR over OSR remain inconclusive.
Endoleak, especially the type II endoleak (T2EL), is a common complication after EVAR. According to research, 16-28% of the patients develop a T2EL after EVAR, and it accounts for nearly three in four of all types of endoleaks. Around 30-50% of the T2EL resolved spontaneously during the follow-up, however, it still causes a secondary intervention in many patients. Therefore, it is critical to monitor endoleaks after repair.
Patent aortic branches in the stent-overlapped area and vasa vasorum have been identified as potential sources of blood flow in T2EL. However, the mechanisms of biological changes or remodeling of the aneurysm sac after the repair are still not clear, but they have been considered to play an important role in the development of endoleaks. Unfortunately, it is impossible to obtain a tissue sample of the aortic wall in patients who underwent EVAR.
MicroRNAs (miRNAs) are a class of small single-stranded non-coding RNAs that inhibit the expression of target message RNA (mRNA). miR-29b/29c, miR-155, and miR-15a are miRNAs associated with regulating extracellular matrix (ECM) components, inflammation, and proliferation, respectively. All four miRNAs have been identified as biomarkers of AAA, not only in aneurysm tissue but also extracellular as circulating miRNAs. However, it is still unknown whether they can reflect the biological changes after AAA repair. Thus, we conducted a prospective study to investigate the changes in expression of circulating miR-29b, miR-29c, miR-155, and miR-15a before (T0), 3 days (T1), and 3 months (T2) after AAA repair.
A total of 39 patients were recruited for this study, 17 of whom were repaired by OSR and 22 of whom were repaired by EVAR. Four patients failed the T2 follow-up due to the Covid-19 pandemic. No significant changes were found in the expression of miR-29b, miR-29c, miR-155, and miR-15a. There were also no obvious differences between OSR and EVAR. However, the T1 expression of miR-15a was significantly lower in patients without endoleak after EVAR than in those who developed endoleak after EVAR and those who were repaired by OSR. Unfortunately, these differences did not persist to the T2 follow-up, and no other differences were found among these patients.
In summary, miR-15a is a miRNA that significantly changes in AAA patients. This study demonstrates that the expression of circulating miR-15a is lower in patients without endoleak three days after EVAR, compared to those who had endoleak after EVAR and those who underwent OSR. The results suggest that miR-15a might be involved in the early aortic remodeling after EVAR as an indicator of endoleak.
Human functional brain connectivity can be temporally decomposed into states of high and low cofluctuation, defined as coactivation of brain regions over time. Rare states of particularly high cofluctuation have been shown to reflect fundamentals of intrinsic functional network architecture and to be highly subject-specific. However, it is unclear whether such network-defining states also contribute to individual variations in cognitive abilities – which strongly rely on the interactions among distributed brain regions. By introducing CMEP, a new eigenvector-based prediction framework, we show that as few as 16 temporally separated time frames (< 1.5% of 10min resting-state fMRI) can significantly predict individual differences in intelligence (N = 263, p < .001). Against previous expectations, individual’s network-defining time frames of particularly high cofluctuation do not predict intelligence. Multiple functional brain networks contribute to the prediction, and all results replicate in an independent sample (N = 831). Our results suggest that although fundamentals of person-specific functional connectomes can be derived from few time frames of highest connectivity, temporally distributed information is necessary to extract information about cognitive abilities. This information is not restricted to specific connectivity states, like network-defining high-cofluctuation states, but rather reflected across the entire length of the brain connectivity time series.
ADHS im Erwachsenenalter ist eine komplexe Erkrankung, die durch verschiedene diagnostische Instrumente erfasst wird. Aktuell spielen dabei vor allem die klinische Exploration, Interview- und Fragebogendaten eine Rolle.
Obwohl bei Patient*innen vielfältige kognitive Einschränkungen bestehen, wird die Verwendung neuropsychologischer Daueraufmerksamkeitstests (CPTs) in den klinischen Leitlinien nur ergänzend erwähnt. Validität und Nützlichkeit von CPTs bei ADHS Patient*innen wird aufgrund der mangelnden Sensitivität und
Spezifität immer wieder kritisch diskutiert. Multidimensionale Ansätze, in denen verschiedene neuropsychologische Beurteilungen mit klinischen Parametern kombiniert werden, sollen das Aufstellen einer ADHS Diagnose im Erwachsenenalter verbessern. Die vorliegende Studie untersucht zwei verschiedene CPTs bezüglich deren Vergleichbarkeit und deren Verknüpfung mit klinischen Parametern. Ein direkter Vergleich innerhalb derselben Stichprobe ist bisher nicht durchgeführt worden, könnte jedoch wichtige Implikationen für die Testauswahl und darauffolgende klinische Interventionen haben. Die klinische Stichprobe besteht aus zufällig ausgewählten erwachsenen Patient*innen, die sich in der ADHS Ambulanz der Klinik für Psychiatrie, Psychosomatik und Psychotherapie des Universitätsklinikums Frankfurt zur diagnostischen ADHS Beurteilung zwischen Mai und November 2019 vorstellten. Die Patient*innen wurden von erfahrenen Ärzt*innen mithilfe des strukturierten diagnostischen Interviews für ADHS bei Erwachsenen (DIVA 2.0) befragt. Au§erdem wurden Schulzeugnisse bewertet und die medizinische Vorgeschichte erhoben. Anschließend wurden die Patient*innen mit dem Wender-Reimherr Interview (WRI) von einer unabhängigen Interviewerin befragt und füllten die folgenden Selbstbeurteilungsfragebögen aus: Kurzform der Wender-Utah Rating Scale (WURS-k), Impulsive Behavior Scale (UPPS) und General Health Questionnaire (GHQ-28). Alle Patient*innen nahmen anschließend an der Testung mit zwei CPTs teil: dem Quantified Behavior Test (Qb-Test) und Nesplora Aquarium [9]. Der Qb-Test misst neben Unaufmerksamkeit und Impulsivität auch Hyperaktivität mithilfe einer Infrarot Kamera und eines Reflektors, welche das Bewegungsmuster während der Testung aufzeichnen. In Nesplora Aquarium wird virtuelle Realität genutzt, um das Bewegungsmuster mithilfe eines optischen Gerätes, das mit Sensoren und Lautsprechern ausgestattet ist, aufzuzeichnen. Es zeigte sich, dass die übergreifende Vergleichbarkeit beider Tests begrenzt ist. Trotz der kongruenten Messung von Unaufmerksamkeit in beiden Tests konnten keine Korrelationen mit den klinischen Parametern festgestellt werden. Hyperaktivität ist in beiden Tests positiv mit aktuellen ADHS Symptomen assoziiert, wobei der Qb-Test darüber hinaus noch Korrelationen mit ADHS Symptomen aus der Kindheit aufweist. Impulsivität stellte sich als ein unabhängiges Konstrukt heraus, sowohl die Korrelation zwischen beiden Tests als auch die Assoziation mit klinischen Parametern betreffend. Dies stellt in Frage, ob ein homogenes mpulsivitätskonstrukt existiert, welches sich in neuropsychologischen Tests und klinischen Symptomen abbildet. Insgesamt ermöglicht die Studie einen guten Vergleich beider CPTs und zeigt detaillierte Informationen über die
Kompatibilität mit klinischen Symptomen auf. Unaufmerksamkeit stellte sich als ein basaler Faktor heraus, der zwischen beiden Tests über alle Patient*innen hinweg überlappt. Da dieser Faktor jedoch nicht mit klinischen Parametern korreliert, unterstreicht dies die Problematik, subjektive mit objektiven Methoden in der ADHS Diagnostik zu verbinden. Die aktuellen Ergebnisse bezweifeln die einfache Anwendung sogenannter objektiver Messverfahren in der klinischen ADHS Diagnostik aufgrund ihrer fragwürdigen Validität. Die Ergebnisse der Studie verdeutlichen die Notwendigkeit eines neuen diagnostischen Ansatzes im Sinne der Präzisionsmedizin, welcher über die konventionellen Klassifikationssysteme hinaus passende individuelle Lösungen für Diagnostik und Behandlung ermöglicht.
In der vorliegenden Dissertation wird auf diesen Ergebnissen basierend der Goldstandard der aktuellen ADHS-Diagnostik unter Beachtung der Leitlinien diskutiert. Die untersuchten CPTs werden unter Berücksichtigung der Gütekriterien guter diagnostischer Verfahren überprüft. Das Potenzial neuropsychologischer Testverfahren im Hinblick auf die Steigerung von Objektivität, Validität und Differenziertheit der Diagnosen wird untersucht und Empfehlungen für die klinische Anwendung formuliert.
The mitochondrial matrix peptidase CLPP is crucial during cell stress. Its loss causes Perrault syndrome type 3 (PRLTS3) with infertility, neurodegeneration, and a growth deficit. Its target proteins are disaggregated by CLPX, which also regulates heme biosynthesis via unfolding ALAS enzymes, providing access for pyridoxal-5′-phosphate (PLP). Despite efforts in diverse organisms with multiple techniques, CLPXP substrates remain controversial. Here, avoiding recombinant overexpression, we employed complexomics in mitochondria from three mouse tissues to identify endogenous targets. A CLPP absence caused the accumulation and dispersion of CLPX-VWA8 as AAA+ unfoldases, and of PLPBP. Similar changes and CLPX-VWA8 co-migration were evident for mitoribosomal central protuberance clusters, translation factors like GFM1-HARS2, the RNA granule components LRPPRC-SLIRP, and enzymes OAT-ALDH18A1. Mitochondrially translated proteins in testes showed reductions to <30% for MTCO1-3, the mis-assembly of the complex IV supercomplex, and accumulated metal-binding assembly factors COX15-SFXN4. Indeed, heavy metal levels were increased for iron, molybdenum, cobalt, and manganese. RT-qPCR showed compensatory downregulation only for Clpx mRNA; most accumulated proteins appeared transcriptionally upregulated. Immunoblots validated VWA8, MRPL38, MRPL18, GFM1, and OAT accumulation. Co-immunoprecipitation confirmed CLPX binding to MRPL38, GFM1, and OAT, so excess CLPX and PLP may affect their activity. Our data mechanistically elucidate the mitochondrial translation fidelity deficits which underlie progressive hearing impairment in PRLTS3.

Hintergrund: Bei steigender Lebenserwartung ergibt sich in Zukunft eine steigende Prävalenz der degenerativen Aortenklappenstenose (AS). Die Transkathether-Aortenklappenimplantation (TAVI) erfährt immer größere Bedeutung in der Behandlung der symptomatischen, hochgradigen AS. Eine paravalvuläre Leckage (PVL) ist ein anerkannter Risikofaktor für eine erhöhte Mortalität und sollte periinterventionell adäquat erkannt und behandelt werden. Eine Postdilatation als typische korrigierende Intervention (CI) kann allerdings zu schwerwiegenden Komplikationen führen. Weitere Instrumente zur Entscheidung über die Notwendigkeit einer CI sind erstrebenswert. Für die Aortenregurgitationsindex-Ratio (ARI-Ratio) und für die präprozedurale Kalklast, gemessen mit der Mehrschicht-Computertomografie (MSCT), wurde gezeigt, dass beide prädiktiv sind für die Notwendigkeit einer periinterventionellen CI.
Ziele: Die ARI-Ratio und der Kalkstatus wurde in Hinblick auf ihren prädiktiven Wert für die Notwendigkeit einer CI miteinander verglichen.
Methoden: Von 199 Patienten nach TAVI erhielten 38,9 % eine Portico™, 29,5 % eine Symetis Acurate™, 21,5 % eine Sapien 3™ und 10,1 % eine Evolut™. Es wurde retrospektiv der postinterventionell erhobene systolische (SB), diastolische (DB) und linksventrikuläre enddiastolische Blutdruck (LVEDP) im ARI zusammengefasst: [(DBP - LVEDP) / SBP] × 100. Die ARI-Ratio wurde berechnet als Quotient zwischen ARI nach und vor dem Eingriff. Des Weiteren wurden die MSCTs mittels „3mensio Structural Heart“ analysiert insbesondere in Hinblick auf die Verkalkung des linksventrikulären Ausflusstrakts, der Aortenklappe (gesamt sowie der einzelnen Taschen) und des sinotubulären Übergang. Dabei wurde der Kalzium-Volumen-Score auf Basis drei verschiedener Thresholds benutzt (500 Hounsfield Einheiten (HU), 800 HU und visuell-individuell). Im Folgenden wurden ROC-Kurven für den ARI-Ratio und für die verschiedenen Kalklastparameter erstellt um die Notwendigkeit einer CI zu beurteilen. Schließlich wurde die Area under the curve (AUC) der ARI-Ratio mit denen der Kalklastsurrogatparameter verglichen.
Ergebnisse: Die ARI-Ratio zeigte mit einer AUC von 0,747 das beste Ergebnis. Bei den Verkalkungsparametern zeigte die Verkalkung der noncoronaren Aortenklappentasche die beste AUC, nachfolgend die der gesamten Aortenklappe. Der 800-HU-Threshold zeigte bessere AUCs als 500 HU. Von 19 Verkalkungssurrogatparametern zeigten elf einen statistisch signifikanten Vorhersagewert auf. Die ARI-Ratio zeigte eine signifikante Überlegenheit gegenüber sechs dieser Parameter. Für die fünf Verbliebenen konnte kein signifikanter Unterschied nachgewiesen werden.
Schlussfolgerung: Die ARI-Ratio ist ein besserer Prädiktor für die Notwendigkeit einer CI als die Kalzifikationsparameter. Auch diverse Kalzifikationsparameter haben diesbezüglich eine Aussagekraft. Die Kalzifikationsparameter können bereits präinterventionell beurteilt werden können, die ARI-Ratio nur periinterventionell. Es sollte neben der Anschauung von bereits etablierten Methoden ein integrativer Ansatz gewählt werden, der Kalklast und ARI-Ratio für die Entscheidung zur Notwendigkeit einer CI miteinschließt. Weitere Studien zur Standardisierung der Verkalkungsparameter sind notwendig für eine weitere Einordnung deren Vorhersagekraft. Es bestanden teils erhebliche Unterschiede zwischen den verwendeten Klappenprothesentypen. Weitere Studien mit einer größeren Anzahl an verschiedenen Prothesentypen könnten richtungsweisende Subgruppenanalysen möglich machen.
Background: The COVID-19 pandemic led to a higher incidence of depression and a worsening of psychiatric conditions, while pre-existing constraints of the healthcare system and safety regulations limited psychiatric care.
Aims: We investigated the impact of the pandemic on the clinical care of patients with a single episode (SE-MDD) or major depressive disorder (MDD) in Germany.
Methods: Nationwide inpatient data were extracted from the German Institute for Hospital Remuneration System for 2020 and 2021 (depression data) and the Robert Koch Institute (COVID-19 incidence). Changes in inpatients were tested with linear regression models. Local cases of depression in our department compared to 2019 were explored with one-way ANOVA and Dunnett's test.
Results: Across Germany, the inpatient numbers with both SE-MDD and MDD declined by more than 50% during three out of four COVID-19 waves. Higher COVID-19 incidence correlated with decreased inpatient numbers. In our department, fewer MDD inpatients were treated in 2020 (adj. p < 0.001) and 2021 (adj. p < 0.001) compared to 2019, while the number of SE-MDD inpatients remained stable. During this period fewer elective and more emergency inpatients were admitted. In parallel, MDD outpatient admissions increased in 2021 compared to 2019 (adj. p = 0.002) and 2020 (adj. p = 0.003).
Conclusion: During high COVID-19 infection rates, MDD patients received less inpatient care, which might cause poor outcomes in the near future. These data highlight the necessity for improved infrastructure in the in- and outpatient domains to facilitate accessibility to adequate care.
A recent in-vivo experiment has shown that force can be transmitted between the gastrocnemius and the hamstring muscles due to a direct tissue continuity. However, it remains unclear if this mechanical interaction is affected by the stiffness of the structural connection. This study therefore aimed to investigate the impact of the knee angle on myofascial force transmission across the dorsal knee. A randomized, cross-over study was performed, including n = 56 healthy participants (25.36 ± 3.9 years, 25 females). On two separate days, they adopted a prone position on an isokinetic dynamometer (knee extended or 60° flexed). In each condition, the device moved the ankle three times from maximal plantarflexion to maximal dorsal extension. Muscle inactivity was ensured using EMG. High-resolution ultrasound videos of the semimembranosus (SM) and the gastrocnemius medialis (GM) soft tissue were recorded. Maximal horizontal tissue displacement, obtained using cross-correlation, was examined as a surrogate of force transmission. SM tissue displacement was higher at extended (4.83 ± 2.04 mm) than at flexed knees (3.81 ± 2.36 mm). Linear regression demonstrated significant associations between (1) SM and GM soft tissue displacement (extended: R2 = 0.18, p = 0.001; flexed: R2 = 0.17, p = 0.002) as well as (2) SM soft tissue displacement and ankle range of motion (extended: R2 = 0.103, p = 0.017; flexed: R2 = 0.095, p = 0.022). Our results further strengthen the evidence that local stretching induces a force transmission to neighboring muscles. Resulting remote exercise effects such as increased range of motion, seem to depend on the stiffness of the continuity.

Purpose: Monocentric, prospective study to investigate whether concomitant support of cochlear implant (CI) patients by CI-trained otolaryngologists and application of a standardized head bandage can minimize potential complications during magnetic resonance imaging (MRI).
Methods: Thirty-seven patients with 46 CIs underwent MRI with a prophylactic head bandage. All participants and the otolaryngologist at the CI center completed pre- and post-MRI questionnaires documenting body region scanned, duration of MRI and bandage wear, field strength during the scan, and any complications. If pain was experienced, it was assessed using a visual analog scale (1–10).
Results: MRI was performed without adverse events in 37.8% of cases. Magnet dislocation requiring surgical revision occurred in 2% of cases. Pain was reported in 86% of cases, often due to the tightness of the dressing. Patients with rotating, MRI-compatible magnets reported significantly less pain than participants with older-generation implants. In 11% of cases, the MRI was discontinued.
Conclusion: Serious complications during MRI in cochlear implant patients are rare. Pain is the most common adverse event, probably mainly due to the tight bandage required by most implant types. With newer generations of magnets, these patients experience less pain, no dislocation of the magnets, and no need for bandaging. Although magnet dislocation cannot be completely prevented in older generations of implants, it appears to be reduced by good patient management, which recommends examination under the guidance of physicians trained in the use of hearing implants.
Hintergrund: Epilepsie bezeichnet eine Erkrankung, welche durch eine anhaltende Prädisposition für Symptome, die im Zusammenhang mit einer ungewöhnlich starken oder synchronisierten elektrischen Aktivität des Gehirns auftreten (=epileptischer Anfall) charakterisiert ist. Mutationen in dem Gen DEPDC5 (Dishevelled, Egl-10 and Pleckstrin (DEP) domain-containing protein 5) sind mit fokalen Epilepsien assoziiert und führen in Tiermodellen und humanen Modellen zu einer Überaktivierung des mTOR-Signalweges. Auf neuromorphologischer Ebene zeigt sich die mTOR-Überaktivierung durch eine Vergrößerung des Zelldurchmessers und einer zunehmenden Verästelung der Neuriten. Ziel dieser Studie war es, die morphologischen Auswirkungen der DEPDC5-assoziierten mTOR-Überaktivierung in der SH-SY5Y-Neuroblastomzelllinie zu untersuchen. Dadurch soll eine Einschätzung getroffen werden, ob das im Zusammenhang mit neurodegenerativen Erkrankungen bereits gut etablierte SH-SY5Y-Zellmodell auch bei der Untersuchung epilepsieassoziierter Pathomechanismen zum Einsatz kommen kann.
Methoden: Unter Einsatz der CRISPR/Cas9-Methode wurden Knockout(KO)-Mutationen in Exon 2 und Exon 3 des DEPDC5-Gens erzeugt und diese mittels Sanger-Sequenzierung bestätigt. Danach wurden die Knockouts auf RNA- und Proteinebene, durch Real-time-RT-PCR und Western Blot validiert. Die bestätigten homozygoten DEPDC5-KO-Zelllinen wurden anschließend mittels Western Blot auf eine mTOR-Überaktivierung untersucht. Zuletzt erfolgte die neuromorphologische Validierung des DEPDC5-KO. Die Zellgröße proliferierender SH-SY5Y wurden mittels Durchflusszytometrie (FACS) untersucht. Zudem bestimmten wir die neuronale Architektur differenzierter SH-SY5Y unter Einsatz der Sholl-Analyse.
Ergebnisse: Es konnten vier unabhängige DEPDC5-KO-SH-SY5Y-Zelllinien mit homozygoten Indel-Mutationen und vorzeitigem Stoppcodon in Exon 3 generiert werden. Die erwartete Reduktion an DEPDC5-mRNA konnte mittels Real-time 10 RT-PCR nicht festgestellt werden. Die Abwesenheit des Proteins konnte durch Western Blot aber gezeigt werden. Funktionell konnte für alle Zelllinien eine mTOR-Überaktivierung mittels Western Blot nachgewiesen werden. Dabei konnte phosphoryliertes AKT (AKT serine/threonine kinase 1) als stabilster Marker etabliert werden. Auf neuromorphologischer Ebene ließ sich ein Trend in Richtung vergrößertem Zelldurchmesser bei verlängertem Auswachsen der Neuriten feststellen, wobei sich für das Modell Unterschiede zwischen den einzelnen Klonen ergaben.
Diskussion: In dieser Studie gelang es erstmals, den Zusammenhang zwischen DEPDC5-KO und einer mTOR-Überaktivierung in der onkogenen SH-SY5Y-Zelllinie zu replizieren. Das verlängerte Auswachsen der Neuriten, bei jedoch gleichbleibender Anzahl peripherer Verästelungen, stellt dabei einen neuen Befund dar und könnte durch die frühe neuronale Entwicklungsstufe des SH-SY5Y-Zellmodells erklärt werden. Auf Grundlage der Ergebnisse dieser Arbeit lässt sich sagen, dass das robuste und kostengünstige SH-SY5Y-Zellmodell insbesondere für high-throughput Methoden und Screeningassays ein geeignetes Modell ist. Durch die Kombination mit reiferen Zellmodellen, wie beispielsweise iPSCs (induced pluripotent stem cells), könnte der Phänotyp eines DEPDC5-KO und anderer mTOR-assoziierter Epilepsien, möglichst umfassend in-vitro dargestellt werden.
Die Krebsstammzellforschung gelangte in den letzten Jahren vermehrt in den Fokus der Tumorforschung. Im Tumor bilden Krebsstammzellen eine kleine Population an Zellen mit Stammzelleigenschaften, wodurch sie eine große Rolle bei der Entstehung von Rezidiven, Metastasen, sowie der Entwicklung von Chemotherapieresistenzen spielen. Um eine gezielte Bekämpfung von Krebsstammzellen zu ermöglichen, müssen diese im
Tumor zunächst zuverlässig durch Krebsstammzellmarker detektiert werden können.
Gerade bei soliden pädiatrischen Tumoren, wie dem Hepatoblastom, ergeben sich hierbei Schwierigkeiten dadurch, dass im sehr heterogenen Tumorgewebe viele Zellen aufgrund der embryonalen Natur des Tumors bereits Stammzellmarker exprimieren, ohne dass es sich bei diesen Zellen um Krebsstammzellen handelt. Das Hepatoblastom ist mit 2/3 der Lebertumore des Kindes die häufigste maligne Leberneoplasie im Kindesalter.
Auch wenn es bereits Hinweise auf das Vorliegen von Krebsstammzellen im Hepatoblastom gibt, so konnten diese bisher nicht genauer durch fest definierte Krebsstammzellmarker identifiziert werden.
Um dies zu erreichen, wurden in dieser Arbeit die beiden Hepatoblastomzelllinien HuH6 und HepG2 auf die Expression der bereits bekannten Krebsstammzellmarker CD90, CD34 und CXCR4 überprüft. Zusätzlich wurde auf eine Bindung des „oval cell“ Antikörpers, OV-6, untersucht. Mittels Durchflusszytometrie-Analysen konnte eine Zellpopulation gefunden werden, welche die Oberflächenmarker CD34 und CD90 koexprimiert und gleichzeitig den OV-6 Antikörper bindet. Im nächsten Schritt wurden die Zellen auf einige Krebsstammzelleigenschaften überprüft. Zur weiteren Untersuchung dieser Subpopulation erfolgte mittels MACS (magnetic activated cell sorting) eine Anreicherung der CD90 exprimierenden Zellen. Diese wurde mittels qPCR auf die Expression der Pluripotenzmarker Oct4 und Nanog, sowie der Zytidindeaminase AID untersucht. Es konnte eine signifikant erhöhte Expression von AID und Oct4 detektiert werden. Im Gegensatz hierzu zeigte sich die Expression von EpCAM, c-myc und Albumin, welche als Kontrollgene untersucht wurden, nicht signifikant erhöht. Um auf das Metastasierungspotential der CD90 angereicherten Zellen rückzuschließen, wurde ein Migrationsassay mit angereicherten und depletierten Zellen durchgeführt. Hier wiesen die CD90 angereicherten Zellen, im Vergleich zu den depletierten Zellen eine erhöhte Migration auf. Im Tumorsphäroid-Assay war die HepG2 Zelllinie in der Lage Tumor-61 -sphäroide auszubilden. Nach der Passagierung zeigten diese eine erhöhte Expression der Krebsstammzellmarker CD90 und CD34, sowie der Pluripotenzmarker Oct4 und Nanog.
Zusammengefasst kann mit den Krebsstammzellmarkern CD90, CD34 und OV-6 eine Subpopulation im Hepatoblastom identifiziert werden, die nach unseren Analysen Krebsstammzelleigenschaften aufweisen. Mithilfe dieses Markersets können nun neue Therapieansätze auf ihre Effektivität, Krebsstammzellen gezielt zu eliminieren, getestet werden.
Gene therapy has garnered increasing interest over recent decades. Several therapies employing gene transfer mechanisms have been developed, and, of these, adeno-associated virus (AAV) vectors have demonstrated viability for use with in vivo gene therapy. Several AAV-based therapeutics have received regulatory approval in the last few years including those for retinal disease, spinal muscular atrophy or aromatic L-amino acid decarboxylase deficiency. Lately, with the introduction of novel liver-directed AAV vector-based therapeutics for the treatment of haemophilia A and B, gene therapy has attracted significant attention in the hepatology community, with the liver increasingly recognised as a target for gene therapy. However, the introduction of foreign DNA into hepatocytes is associated with a risk of hepatic reactions, with raised ALT (alanine aminotransferase) and AST (aspartate aminotransferase) being – so far – the most commonly reported side effects. The complete mechanisms underlying the ALT flairs remain to be determined and the long-term risks associated with these new treatments is not yet known. The liver community is increasingly being asked to support liver-directed gene therapy to mitigate potential liver associated harm. In this review, we focus on AAV vector-based gene therapy, shedding light on this promising technique and its remarkable success in haemophilia, with a special focus on hepatic complications and their management in daily clinical practice.
Branching allows neurons to make synaptic contacts with large numbers of other neurons, facilitating the high connectivity of nervous systems. Neuronal arbors have geometric properties such as branch lengths and diameters that are optimal in that they maximize signaling speeds while minimizing construction costs. In this work, we asked whether neuronal arbors have topological properties that may also optimize their growth or function. We discovered that for a wide range of invertebrate and vertebrate neurons the distributions of their subtree sizes follow power laws, implying that they are scale invariant. The power-law exponent distinguishes different neuronal cell types. Postsynaptic spines and branchlets perturb scale invariance. Through simulations, we show that the subtree-size distribution depends on the symmetry of the branching rules governing arbor growth and that optimal morphologies are scale invariant. Thus, the subtree-size distribution is a topological property that recapitulates the functional morphology of dendrites.
Traditionally, biosensors are designed to detect one specific analyte. Nevertheless, disease progression is regulated in a highly interactive way by different classes of biomolecules like proteins and nucleic acids. Therefore, a more comprehensive analysis of biomarkers from a single sample is of utmost importance to further improve both, the accuracy of diagnosis as well as the therapeutic success. This review summarizes fundamentals like biorecognition and sensing strategies for the simultaneous detection of proteins and nucleic acids and discusses challenges related to multianalyte biosensor development. We present an overview of the current state of biosensors for the combined detection of protein and nucleic acid biomarkers associated with widespread diseases, among them cancer and infectious diseases. Furthermore, we outline the multianalyte analysis in the rapidly evolving field of single-cell multiomics, to stress its significance for the future discovery and validation of biomarkers. Finally, we provide a critical perspective on the performance and translation potential of multianalyte biosensors for medical diagnostics.
An 80-year-old post–coronary artery bypass graft (CABG) patient had an acute coronary syndrome with non–ST-segment elevation myocardial infarction (ACS-NSTE) with saphenous vein graft (SVG)–obtuse marginal stenosis. High-definition intravascular ultrasound revealed an underexpanded SVG stent with a hyperechoic structure. Optical coherence tomography confirmed surgical clip causing compression, resolved by post-dilation. This case underscores ACS-NSTE complexity post-CABG and the critical role of coronary imaging in optimizing interventions by addressing surgical clip–induced compression.

Abstract
Beside its involvement in somatic dysfunctions, altered insulin signalling constitutes a risk factor for the development of mental disorders like Alzheimer’s disease and obsessive-compulsive disorder. While insulin-related somatic and mental disorders are often comorbid, the fundamental mechanisms underlying this association are still elusive. Studies conducted in rodent models appear well suited to help decipher these mechanisms. Specifically, these models are apt to prospective studies in which causative mechanisms can be manipulated via multiple tools (e.g., genetically engineered models and environmental interventions), and experimentally dissociated to control for potential confounding factors. Here, we provide a narrative synthesis of preclinical studies investigating the association between hyperglycaemia – as a proxy of insulin-related metabolic dysfunctions – and impairments in working and spatial memory, and attention. Ultimately, this review will advance our knowledge on the role of glucose metabolism in the comorbidity between somatic and mental illnesses.

Abstract
Background: We aimed to investigate whether combined phosphorous (31P) magnetic resonance spectroscopic imaging (MRSI) and quantitative T′2 mapping are able to detect alterations of the cerebral oxygen extraction fraction (OEF) and intracellular pH (pHi) as markers the of cellular energy metabolism in cerebral small vessel disease (SVD).
Materials and methods: 32 patients with SVD and 17 age-matched healthy control subjects were examined with 3-dimensional 31P MRSI and oxygenation-sensitive quantitative T′2 mapping (1/T′2 = 1/T2* - 1/T2) at 3 Tesla (T). PHi was measured within the white matter hyperintensities (WMH) in SVD patients. Quantitative T′2 values were averaged across the entire white matter (WM). Furthermore, T′2 values were extracted from normal-appearing WM (NAWM) and the WMH and compared between patients and controls.
Results: Quantitative T′2 values were significantly increased across the entire WM and in the NAWM in patients compared to control subjects (149.51 ± 16.94 vs. 138.19 ± 12.66 ms and 147.45 ± 18.14 vs. 137.99 ± 12.19 ms, p < 0.05). WM T′2 values correlated significantly with the WMH load (ρ=0.441, p = 0.006). Increased T′2 was significantly associated with more alkaline pHi (ρ=0.299, p < 0.05). Both T′2 and pHi were significantly positively correlated with vascular pulsatility in the distal carotid arteries (ρ=0.596, p = 0.001 and ρ=0.452, p = 0.016).
Conclusions: This exploratory study found evidence of impaired cerebral OEF in SVD, which is associated with intracellular alkalosis as an adaptive mechanism. The employed techniques provide new insights into the pathophysiology of SVD with regard to disease-related consequences on the cellular metabolic state.
The question of whether nuclear energy—as a source with relatively low carbon dioxide emissions—can be classified as a sustainable energy source has come into focus in connection with climate change. There is a controversy over securing independence from fossil fuels and gas supplies from other countries through a revival of nuclear energy. On the other hand, some viewpoints are critical: the handling of nuclear waste and the still unclear risks to human health and the environment, especially in light of recent perils from Russian military attacks on Ukrainian nuclear plants. To evaluate the worldwide publications on nuclear energy under health and environmental aspects, socio-economic parameters were included to provide an informed background for all stakeholders, from scientists to decision-makers. The correlation between the number of nuclear power plants and the publication output of the countries is proven to be highly significant. Thus, the operating countries publish the most. It has been shown that the development and economic use of nuclear energy are major stimuli for scientific endeavors. Reactor accidents have also spurred research. Mathematical risk modeling has been the area with the highest citation rate to date, but environmental and health aspects have become more important, especially after major accidents. The results show the importance of economic interests in research on nuclear energy from health and environmental aspects. Against the background of transnational hazards, global research participation should be encouraged. Moreover, the international debate should not ignore the reality of threats and their possible impacts.
Despite antagonizing attempts from the tobacco industry, passive inhalation of tobacco smoke is known to be cancerogenic and toxic to human health for decades. Nonetheless, millions of non-smoking adults and children are still victims of second-hand smoke. Accumulation of particulate matter (PM) in confined spaces such as the car are particularly harmful due to high concentrations. We here aimed to analyze the specific effects of ventilation conditions in the setting of a car. By the use of the measuring platform TAPaC (tobacco-associated particulate matter emissions inside a car cabin), 3R4F reference cigarettes, Marlboro red, and Marlboro gold were smoked in a car interior with a volume of 3.709 m3. Seven different ventilation conditions (C1–C7) were analyzed. Under C1, all windows were closed. Under C2–C7, the car ventilation was turned on power level 2/4 with the air directed towards the windshield. Only the passenger side window was opened, where an outer placed fan could create an airstream speed of 15.9–17.4 km/h at one meter distance to simulate a driving car. C2: Window 10 cm opened. C3: Window 10 cm opened with the fan turned on. C4: Window half-opened. C5: Window half-opened with the fan turned on. C6: Window fully opened. C7: Window fully opened with the fan turned on. Cigarettes were remotely smoked by an automatic environmental tobacco smoke emitter and a cigarette smoking device. Depending on the ventilation condition the cigarettes emitted different mean PM concentrations after 10 min under condition C1 (PM10: 1272–1697 µg/m3, PM2.5: 1253–1659 µg/m3, PM1: 964–1263 µg/m3) under C2, C4, and C6 (PM10: 68.7–196.2 µg/m3, PM2.5: 68.2–194.7 µg/m3, PM1: 66.1–183.8 µg/m3) C3, C5, and C7 (PM10: 73.7–139 µg/m3, PM2.5: 72–137.9 µg/m3, PM1:68.9–131.9 µg/m3). Vehicle ventilation is insufficient to protect passengers from toxic second-hand smoke completely. Brand-specific variations of tobacco ingredients and mixtures markedly influence PM emissions under ventilation conditions. The most efficient ventilation mode to reduce PM exposure was achieved by opening the passenger´s window 10 cm and turning the onboard ventilation on power level 2/4. In-vehicle smoking should be banned to preserve innocent risk groups (e.g., children) from harm.
The combination of pamapimod and pioglitazone (KIN001) has a synergetic antiviral, anti-inflammatory, and antifibrotic activity, which may prevent evolution toward COVID-19-associated severe respiratory failure. In a randomized, placebo-controlled, double-blind, phase 2, multicenter trial, 128 non-critically ill hospitalized patients with confirmed COVID-19 were treated with KIN001 or a placebo for 28 days. The proportion of patients alive and free of oxygen or respiratory support at the end of the therapy was lower than anticipated but not different in the two groups (KIN001 n = 19, 29%, placebo n = 21, 33%). 85 participants had at least one adverse event, with no difference in the number and distribution of events between the two groups. The clinical trial was stopped for futility, mainly due to a lower-than-expected incidence of the primary endpoint. KIN001 was safe and well-tolerated but had no significant effect on clinical outcome.
Fondaparinux sodium is a chemically synthesized selective factor Xa inhibitor approved for the prevention and treatment of venous thromboembolic events, that is, deep vein thrombosis, pulmonary embolism, and superficial vein thrombosis, in acutely ill (including those affected by COVID-19 or cancer patients) and those undergoing surgeries. Since its approval in 2002, the efficacy and safety of fondaparinux is well demonstrated by many clinical studies, establishing the value of fondaparinux in clinical practice. Some of the advantages with fondaparinux are its chemical nature of synthesis, minimal risk of contamination, 100% absolute bioavailability subcutaneously, instant onset of action, a long half-life, direct renal excretion, fewer adverse reactions when compared with direct oral anticoagulants, and being an ideal alternative in conditions where oral anticoagulants are not approved for use or in patients intolerant to low molecular weight heparins (LMWH). In the last decade, the real-world use of fondaparinux has been explored in other conditions such as acute coronary syndromes, bariatric surgery, in patients developing vaccine-induced immune thrombotic thrombocytopenia (VITT) and in pregnant women with heparin-induced thrombocytopenia (HIT), or those intolerant to LMWH. The emerging data from these studies have culminated in recent updates in the guidelines that recommend the use of fondaparinux under various conditions. This paper aims to review the recent data and the subsequent updates in the recommendations of various guidelines on the use of fondaparinux sodium.
Background/Objectives: Agility and cognitive abilities are typically assessed separately by different motor and cognitive tests. While many agility tests lack a reactive decision-making component, cognitive assessments are still mainly based on computer-based or paper-pencil tests with low ecological validity. This study is the first to validate the novel SKILLCOURT technology as an integrated assessment tool for agility and cognitive-motor performance.
Methods: Thirty-two healthy adults performed agility (Star Run), reactive agility (Random Star Run) and cognitive-motor (executive function test, 1-back decision making) performance assessments on the SKILLCOURT. Cognitive-motor tests included lower limb responses in a standing position to increase the ecological validity when compared to computer-based tests. Test results were compared to established motor and agility tests (countermovement jump, 10 m linear sprint, T-agility tests) as well as computer-based cognitive assessments (choice-reaction, Go-NoGo, task switching, memory span). Correlation and multiple regression analyses quantified the relation between SKILLCOURT performance and motor and cognitive outcomes.
Results: Star Run and Random Star Run tests were best predicted by linear sprint (r = 0.68, p < 0.001) and T-agility performance (r = 0.77, p < 0.001), respectively. The executive function test performance was well explained by computer-based assessments on choice reaction speed and cognitive flexibility (r = 0.64, p < 0.001). The 1-back test on the SKILLCOURT revealed moderate but significant correlations with the computer-based assessments (r = 0.47, p = 0.007).
Conclusion: The results support the validity of the SKILLCOURT technology for agility and cognitive assessments in more ecologically valid cognitive-motor tasks. This technology provides a promising alternative to existing performance assessment tools.
Hintergrund: Anämie gehört zu den häufigsten Erkrankungen weltweit und stellt daher ein zentrales globales Gesundheitsproblem dar. Etwa ein Drittel aller chirurgischen Patienten weisen präoperativ eine Anämie auf. Dies wird als unabhängiger Risikofaktor für eine erhöhte Morbidität und Mortalität sowie für das vermehrte Auftreten postoperativer Komplikationen angesehen. Vor diesem Hintergrund wurde von der Weltgesundheitsorganisation (World Health Organization, WHO) ein patientenzentriertes Behandlungskonzept namens Patient Blood Management (PBM) gefordert, um unter anderem Anämien frühzeitig zu diagnostizieren und einen rationalen Einsatz von Fremdblutprodukten zu fördern. Trotz der Tatsache, dass PBM vor etwa 10 Jahren eingeführt wurde, scheint Deutschland im internationalen Vergleich eine überdurchschnittlich hohe Menge an Erythrozytenkonzentrat-(EK)Einheiten pro Kopf zu transfundieren. Die Ursachen dafür sind bislang unklar und könnten möglicherweise durch eine erhöhte Anämie- Prävalenz bei chirurgischen Patienten und dem damit verbundenen erhöhten Bedarf an Transfusionen einhergehen.
Zielsetzung: Ziel dieser Arbeit ist die multizentrische Erfassung der präoperativen Anämie-Prävalenz innerhalb des Zeitraums 2007-2019 in Deutschland.
Methoden: In dieser retrospektiven, multizentrischen Beobachtungsstudie wurden alle Patienten jeden Alters, die im Monat März der Jahre 2007, 2012, 2015, 2017 und 2019 in acht teilnehmenden Krankenhäusern operiert wurden, erfasst. Patientencharakteristika und klinische Daten wurden aus den Krankenhausinformationssystemen der teilnehmenden Krankenhäuser entnommen. Primäres Ziel war die Prävalenz der Anämie bei Krankenhausaufnahme. Sekundäre Endpunkte waren der Zusammenhang zwischen Anämie und Anzahl der transfundierten EKs, Dauer des Krankenhausaufenthalts und Sterblichkeit im Krankenhaus.
Ergebnisse: Von insgesamt 30.763 Patienten konnten 23.836 Patienten aus acht Krankenhäusern in die Analyse eingeschlossen werden. Im untersuchten Zeitraum zeigte sich eine Reduktion der präoperativen Anämie-Prävalenz erwachsener Patienten von 37% auf 32,2%. Die Zahl, der mit Erythrozytenkonzentraten -5- transfundierten Patienten, ging signifikant von 16,5 % im Jahr 2007 auf 7,8 % im Jahr 2019 zurück, wobei Patienten mit präoperativer Anämie im Vergleich zu Patienten ohne Anämie sechsmal mehr EKs erhielten. Insgesamt verkürzte sich die Krankenhausverweildauer seit 2007 deutlich, zeigte jedoch eine signifikante Verlängerung bei Vorliegen einer präoperativen Anämie. Die Sterblichkeitsrate war über die Jahre hinweg konstant, dennoch signifikant höher bei anämischen Patienten. Eine multivariate Regressionsanalyse mit festen Effekten ergab, dass präoperative Anämie und EK-Transfusion Prädiktoren für die Sterblichkeitsrate waren.
Diskussion: Die Prävalenz der präoperativen Anämie lag in unserer Studienpopulation im Jahr 2019 bei 32,2 %, was der weltweiten Prävalenz entspricht. Das Implementieren von PBM-Strategien, um präoperative Anämien frühzeitig zu identifizieren und zu therapieren sowie Blutprodukte rational einzusetzen, nimmt daher weiterhin einen großen Stellenwert ein. Diese perioperativen Maßnahmen sind für alle chirurgischen Patienten von zentraler Bedeutung, da eine präoperative Anämie den Bedarf an EK-Transfusionen erhöhen, die Liegezeit verlängern und mit einer erhöhten Sterblichkeitsrate assoziiert sein kann.
Traditionally, biosensors are designed to detect one specific analyte. Nevertheless, disease progression is regulated in a highly interactive way by different classes of biomolecules like proteins and nucleic acids. Therefore, a more comprehensive analysis of biomarkers from a single sample is of utmost importance to further improve both, the accuracy of diagnosis as well as the therapeutic success. This review summarizes fundamentals like biorecognition and sensing strategies for the simultaneous detection of proteins and nucleic acids and discusses challenges related to multianalyte biosensor development. We present an overview of the current state of biosensors for the combined detection of protein and nucleic acid biomarkers associated with widespread diseases, among them cancer and infectious diseases. Furthermore, we outline the multianalyte analysis in the rapidly evolving field of single-cell multiomics, to stress its significance for the future discovery and validation of biomarkers. Finally, we provide a critical perspective on the performance and translation potential of multianalyte biosensors for medical diagnostics.
Approximately 80 % of persistent wound infections are affected by the presence of bacterial biofilms, resulting in a severe clinical challenge associated with prolonged healing periods, increased morbidity, and high healthcare costs. Unfortunately, in vitro models for wound infection research almost exclusively focus on early infection stages with planktonic bacteria. In this study, we present a new approach to emulate biofilm-infected human wounds by three-dimensional human in vitro systems. For this purpose, a matured biofilm consisting of the clinical key wound pathogen Pseudomonas aeruginosa was pre-cultivated on electrospun scaffolds allowing for non-destructive transfer of the matured biofilm to human in vitro wound models. We infected tissue-engineered human in vitro skin models as well as ex vivo human skin explants with the biofilm and analyzed structural tissue characteristics, biofilm growth behavior, and biofilm-tissue interactions. The structural development of biofilms in close proximity to the tissue, resulting in high bacterial burden and in vivo-like morphology, confirmed a manifest wound infection on all tested wound models, validating their applicability for general investigations of biofilm growth and structure. The extent of bacterial colonization of the wound bed, as well as the subsequent changes in molecular composition of skin tissue, were inherently linked to the characteristics of the underlying wound models including their viability and origin. Notably, the immune response observed in viable ex vivo and in vitro models was consistent with previous in vivo reports. While ex vivo models offered greater complexity and closer similarity to the in vivo conditions, in vitro models consistently demonstrated higher reproducibility. As a consequence, when focusing on direct biofilm-skin interactions, the viability of the wound models as well as their advantages and limitations should be aligned to the particular research question of future studies. Altogether, the novel model allows for a systematic investigation of host-pathogen interactions of bacterial biofilms and human wound tissue, also paving the way for development and predictive testing of novel therapeutics to combat biofilm-infected wounds.
Einleitung: Im Jahr 2016 wurde eine neue, individuelle Lehrform, das „Lernzentrum für ein individualisiertes medizinisches Tätigkeitstraining und Entwicklung“ („Limette“), an der Westfälischen Wilhelms-Universität Münster entwickelt. Das Ziel dieser Lernform ist und war es, nicht nur die theoretische Lehre, sondern vor allem die praktische Lehre und praktische Szenarien in einem geschütztem Lernumfeld in der medizinischen Lehrdidaktik in den Vordergrund zu stellen.
Die Studierenden nehmen an verschiedenen Fächern der Limette teil. In dieser Promotionsarbeit liegt der Fokus auf der Limette Patient Blood Management (PBM) und Transfusionsmedizin.
Vor Kursbeginn wird den Studierenden Zugang zu verschiedenen Vorbereitungsvideos sowie weiteren theoretischen Materialien über PBM und Transfusionsmedizin zur Verfügung gestellt. Bei der Limette durchläuft jeder Studierende sechs verschiedene Stationen. Hierbei beobachten Dozenten die Fertigkeiten und die Umsetzung der Aufgabenlösung durch halbverspiegelte Scheiben, im Umgang mit echten Blutprodukten oder die Kommunikation mit Patienten, die von Schauspielern dargestellt werden. Die Beobachtungen werden den Dozenten des Seminars, welches im Anschluss zur Nachbesprechung stattfindet, mitgeteilt. Hier teilen die Studierenden untereinander ihr Wissen, können den Dozenten Fragen stellen und werden ihnen weitere theoretisch-praktische Kenntnisse vermittelt.
Ziel der Arbeit: Die Promotionsarbeit hat die Auswertung der Selbsteinschätzungsbögen vor und nach der Limette mit anschließender Interpretation der Ergebnisse zum Ziel. Es soll eruiert werden, ob diese neue Lehrmethodik effektiv in der Umsetzung des Lehrerfolgs ist.
Methodik: Die Selbsteinschätzungsbögen hinsichtlich medizinischer Fertigkeiten und Tätigkeiten werden vor und nach der Limette von den Studierenden ausgefüllt. Die Aufgabenstellungen fokussieren sich jeweils auf medizinische Tätigkeiten, bei denen aus 13 verschiedenen Entrustable Professional Activities (EPAs) diejenigen hinterlegt werden, die bei dieser Aufgabe gelehrt werden. Die Differenz von post- zu prä- Selbsteinschätzung zeigt den Lehrerfolg. Die EPAs werden alle analysiert und mit Hilfe der sogenannten Cohen-Zahl interpretiert. Die Cohen Zahl ist definiert als die Differenz zwischen den Mittelwerten zweier Gruppen dividiert durch die gemeinsame Standardabweichung der beiden Populationen. Nach dem Autor Hattie wurde die Skalierung von Cohen für die Lehre angepasst und dabei folgende Einteilung definiert: d = 0,2 (klein), d = 0,4 (mittel) und d = 0,6 (groß). Alle Methoden mit d > 0,4 wurden als „Zone der gewünschten Effekte“ eingestuft.
Ergebnisse: Im Wintersemester 2021/2022 nahmen n=122 Studierende teil, von welchen n=50 beide Fragebögen komplettierten. Eine signifikante Zahl der dargestellten EPAs (50%) hat gute Ergebnisse erzielt, weist mithin einen Cohen´s Wert von d > 0,40 auf. Im Sommersemester 2022 nahmen n=99 Studierende teil, wobei nur n=29 beide Selbsterhebungsbögen komplettierten. Aufgrund des niedrigen Rücklaufs war die Zahl der ausgefüllten Selbsteinschätzungsbögen deutlich geringer und zugleich die Ergebnisse geringer ausgeprägt. In jenem Semester weist die Mehrheit der Ergebnisse einen Cohen´s d-Wert zwischen 0,15 und 0,4 auf, was als durchschnittlicher Lehrerfolg beurteilt wird.
Schlussfolgerung: Die Lehrform der Limette ist eine neue Form der praktischen Lehrvermittlung, die kontinuierlichen Anpassungen unterliegt. Eine hohe Zahl an ausgefüllten Selbsteinschätzungsbögen ist erstrebenswert, um den Lehrerfolg positiv und statistisch signifikant interpretieren zu können. Hierbei ist es nötig, die Lehre in der Limette auszuwerten und gegebenenfalls weitere Anpassungen durchzuführen.
Die Limette ist eine neue Lehrmethode mit einem guten Lehrerfolg hinsichtlich der Vermittlung praktischer Fähigkeiten und sozialer Kompetenz, welche auch an anderen Institutionen angewandt und umgesetzt werden könnte.
Beside its involvement in somatic dysfunctions, altered insulin signalling constitutes a risk factor for the development of mental disorders like Alzheimer’s disease and obsessive-compulsive disorder. While insulin-related somatic and mental disorders are often comorbid, the fundamental mechanisms underlying this association are still elusive. Studies conducted in rodent models appear well suited to help decipher these mechanisms. Specifically, these models are apt to prospective studies in which causative mechanisms can be manipulated via multiple tools (e.g., genetically engineered models and environmental interventions), and experimentally dissociated to control for potential confounding factors. Here, we provide a narrative synthesis of preclinical studies investigating the association between hyperglycaemia – as a proxy of insulin-related metabolic dysfunctions – and impairments in working and spatial memory, and attention. Ultimately, this review will advance our knowledge on the role of glucose metabolism in the comorbidity between somatic and mental illnesses.
Im 20. Jahrhundert entwickelte sich die Kinderchirurgie aus der Allgemeinchirurgie als eigenständiges Fachgebiet, da zunehmend klar wurde, dass Kinder einer anderen Behandlung bedürfen als Erwachsene und andere Rahmenbedingungen benötigen. Während sich die Erwachsenenchirurgie danach in immer mehr Subdisziplinen aufteilte, ist die Kinderchirurgie ein generalistisches chirurgisches Fach geblieben.
Die vorliegende Arbeit analysiert die Entwicklung der Kinderchirurgie in Deutschland im Hinblick auf medizinische und nicht-medizinische Aspekte und die aktuelle Versorgungssituation, letzteres mit einem Vergleich der Ergebnisqualität ausgewählter angeborener Fehlbildungen im internationalen Vergleich.
Für die Analyse wurden Methoden der Versorgungsforschung angewandt. Die beinhaltete insbesondere die Nutzung von öffentlichen Statistiken und Sekundärdaten, die Statistiken der Deutschen Gesellschaft für Kinderchirurgie sowie die Befragung von ausgewählten Gruppen. Die Analyse bezog sich im Detail auf die Anzahl der Einrichtungen, die akademische und nicht akademische Ausrichtung der Leistungserbringer, die regionale Verteilung, die Personalentwicklung, die Verteilung von Patientinnen und Patienten zwischen den Fachgebieten und innerhalb der Kinderchirurgie und die Netzwerkbildung. Weiterhin wurde die Ergebnisqualität ausgewählter angeborener Fehlbildungen im internationalen Kontext analysiert.
Die Kinderchirurgie ist in Deutschland die Hauptdisziplin für die chirurgische Behandlung von Kindern und Jugendlichen geworden. Sie hat eine weitgehende Flächendeckung erreicht. Die Entwicklung wurde dabei von politischen Entscheidungen beeinflusst, was am Beispiel der Stagnation der Niederlassungen und der Zunahme der kinderchirurgischen Einrichtungen nach Änderung der Perinatal-Versorgung gezeigt werden konnte. Das Spektrum der Kinderchirurgie umfasst die allgemeine Kinderchirurgie, die Neugeborenenchirurgie, Kinderurologie, Kindertraumatologie, Thoraxchirurgie, Teile der Kinderneurochirurgie, plastische Kinderchirurgie, operative Kinderonkologie und die Verbrennungsmedizin. Sie geht damit weit über die Versorgung von angeborenen Fehlbildungen hinaus, wenngleich diese im Zentrum der politischen Diskussionen stehen. Die Arbeitsbedingungen in der Kinderchirurgie haben sich in den letzten Jahren konstant geändert. Die Zahl der Kliniken hat kontinuierlich zugenommen, genauso wie die Zahl der Fachärztinnen und Fachärzte. Der Anteil an Frauen und an Teilzeitbeschäftigten stieg ebenfalls. Die Zahl der ausgeschriebenen Stellen nahm in den Jahren 2021 und 2022 deutlich zu. Die Patientenzahl in den Abteilungen nahm insbesondere seit 2017 ab.
Für die Analyse der Ergebnisqualität wurden die Krankheitsbilder Ösophagusatresie, Dünndarmatresie, Colonatresie, Bauchwanddefekte und Zwerchfellhernien gewählt. Für Kinder mit Ösophagusatresie und Bauchwanddefekten konnte aufgrund der spezifischen Prozeduren die dezentrale Versorgung für Deutschland aufgezeigt werden. Der Vergleich der Ergebnisqualität der Versorgung von angeborenen Fehlbildungen zeigte keine schlechteres Mortalität bei den operierten Kindern mit Ösophagusatresie, den Kindern mit Bauchwanddefekten, Zwerchfellhernie und Darmatresien. Die Verweildauer entsprach bei der Ösophagusatresie, den Bauchwanddefekten und Zwerchfellhernie dem internationalen Vergleich, bei den Darmatresien war sie länger. Weiterhin gab es eine vermehrte Anlage von Stomata bei den Darmatresien.
Die Auswertung der Perinatalzentren Level 1 und 2 ergab eine konstant große Anzahl von Zentren ohne Kinderchirurgie am eigenen Standort. Im Bereich der seltenen Erkrankungen zeigte die Zuordnung der Kliniken zu den verschiedenen Zentren keine durchgehende Systematik. Das Gütesiegel „Ausgezeichnet für Kinder“ hatten 26 kinderchirurgische Kliniken, das Gütesiegel „Das schwerbrandverletzte Kind“ erhielten 16 Zentren für schwerbrandverletzte Kinder, 4 Erwachsenenzentren, die auch Kinder behandelten und 6 spezialisierte Kliniken für brandverletzte Kinder.
Zusammenfassend zeigt die Arbeit die erreichte Flächendeckung der Kinderchirurgie, die teilweise durch politische Entscheidungen geprägt ist. Das Spektrum ist weiterhin generalistisch geblieben, auch wenn nicht jede Abteilung alle Anteile des Spektrums abdeckt. Die Versorgung von angeborenen Fehlbildungen erfolgt grundsätzlich dezentral.
Human feline leukaemia virus subgroup C receptor-related proteins 1 and 2 (FLVCR1 and 2) are major facilitator superfamily transporters from the solute carrier family 49. Dysregulation of these ubiquitous transporters has been linked to various haematological and neurological disorders. While both FLVCRs were initially proposed to hold a physiological function in heme transport, subsequent studies questioned this notion. Here, we used structural, computational and biochemical methods and conclude that these two FLVCRs function as human choline transporters. We present cryo-electron microscopy structures of FLVCRs in different inward- and outward-facing conformations, captured in the apo state or in complex with choline in their translocation pathways. Our findings provide insights into the molecular framework of choline coordination and transport, largely mediated by conserved cation-π interactions, and further illuminate the conformational dynamics of the transport cycle. Moreover, we identified a heme binding site on the protein surface of the FLVCR2 N-domain, and observed that heme actively drives the conformational transitions of the protein. This auxiliary binding site might indicate a potential regulatory role of heme in the FLVCR2 transport mechanisms. Our work resolves the contested substrate specificity of the FLVCRs, and sheds light on the process of maintaining cellular choline homeostasis at the molecular level.
Der Einfluss von Trauma-aktiviertem platelet-rich fibrin auf mesenchymale Stammzellen in vitro

Ziel dieser Arbeit war es, die Wechselwirkung zwischen MSC und PRF von Traumapatienten herauszuarbeiten und zu überprüfen, ob ein therapeutischer Einsatz der Kombination von MSC und Trauma-aktiviertem PRF in Frakturen sinnvoll erscheint.
Hierfür wurden MSC mit PRF über einen Zeitraum von 24 oder 120 Stunden co-inkubiert und die metabolische Aktivität, definierte Faktoren im Überstand (IL-6, CXCL10, VEGF und IDO-2) und die Genexpression ausgewählter Faktoren (MAPK8, MAPK14, IL-6, CXCL10,IDO-1, TNFAIP6, VEGFA, RUNX2 und COL1A) in bis zu drei Messzeitpunkten überprüft. Die Versuche erfolgten vergleichend mit Traumapatienten und gesunden Probanden.
Es konnte gezeigt werden, dass Trauma-aktiviertes PRF verglichen mit PRF von gesunden Probanden die metabolische Aktivität von MSC nach 120 Stunden erhöhen kann. Zudem stellten sich erhöhte Werte des inflammatorischen Faktors IL-6 im Überstand bei Traumapatienten nach 24 und 72 Stunden dar, welche bis zum dritten Messzeitpunkt nach 120 Stunden wieder abfielen. Ein ähnlicher Verlauf zeigte sich bei den Messwerten von VEGFA, einem Faktor, welcher eine große Rolle bei der Angiogenese spielt.
Über alle Messzeitpunkte hinweg konnten niedrigere Werte in der Genexpression von COL1A und RUNX2 im Vergleich zu den Kontrollmessungen erfasst werden.
Die Ergebnisse zeigen, dass im Vergleich zu PRF gesunder Probanden Trauma-aktiviertes PRF die Möglichkeit besitzt, die Proliferation von MSC zu stimulieren. Außerdem wurde beobachtet, dass Trauma-aktiviertes PRF ein inflammatorisches und angiogenetisches Potenzial nach 72 Stunden besitzt, welches nach 120 Stunden allerdings wieder abfällt. Möglicherweise kann durch den Einfluss von MSC der Inflammation entgegengewirkt werden. Ein signifikantes osteogenes Potenzial des Trauma-aktivierten PRFs konnte zu keinem Messzeitpunkt nachgewiesen werden.
Inwiefern der Einsatz von MSC in Kombination mit Trauma-aktiviertem PRF die Frakturheilung verbessern kann, konnte in der vorliegenden Studie nicht eindeutig geklärt werden. Es kann allerdings davon ausgegangen werden, dass die Implantation von Trauma-aktiviertem PRF und MSC in den Defekt erst nach Verstreichen einer bestimmten Zeitperiode nach dem Trauma durchgeführt werden sollte. Die Gründe für diese Annahme sind das erhöhte proliferative und verringerte inflammatorische Potenzial im Verlauf. Eine Ursache für das Fehlen der osteogenen Differenzierung der MSC kann der kurze Messzeitraum sein.
Weitere Studien sollten folgen, um das Potenzial der Kombination von MSC und Trauma-aktiviertem PRF als therapeutisches Verfahren zu überprüfen.

The ongoing COVID-19 pandemic, caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is partly under control by vaccination. However, highly potent and safe antiviral drugs for SARS-CoV-2 are still needed to avoid development of severe COVID-19. We report the discovery of a small molecule, Z-Tyr-Ala-CHN2, which was identified in a cell-based antiviral screen. The molecule exerts sub-micromolar antiviral activity against SARS-CoV-2, SARS-CoV-1, and human coronavirus 229E. Time-of-addition studies reveal that Z-Tyr-Ala-CHN2 acts at the early phase of the infection cycle, which is in line with the observation that the molecule inhibits cathepsin L. This results in antiviral activity against SARS-CoV-2 in VeroE6, A549-hACE2, and HeLa-hACE2 cells, but not in Caco-2 cells or primary human nasal epithelial cells since the latter two cell types also permit entry via transmembrane protease serine subtype 2 (TMPRSS2). Given their cell-specific activity, cathepsin L inhibitors still need to prove their value in the clinic; nevertheless, the activity profile of Z-Tyr-Ala-CHN2 makes it an interesting tool compound for studying the biology of coronavirus entry and replication.
Trauma ist in Deutschland und weltweit eine der häufigsten Todesursache bei Personen unter 55 Jahren. Eine traumatische Verletzung von Gewebe führt zur Freisetzung von sogenannten damage-associated molecular patterns (DAMPs), die eine Entzündungskaskade auslösen, welche die Organfunktion negativ beeinträchtigt. Dies führt bei anhaltender Entzündung zunächst zu Organdysfunktion, was im weiteren Verlauf zu Organversagen führt und im Spätstadium im multiplen Organversagen (MOF) enden kann. In den meisten Fällen ist die inflammatorische Antwort des Immunsystems auf das Trauma adäquat und entspricht einem gut koordinierten Netzwerk von Immunzellen, Zytokinen und Chemokinen, welches zur Wiederherstellung des geschädigten Gewebes führt. Wenn dieses Netzwerk jedoch nicht im Gleichgewicht ist, kann die Entzündungsreaktion durch eine sogenannte feed-forward-loop von Inflammation und Gewebeschäden verstärkt werden. Wenn dieser Prozess systemisch wird, spricht man vom systemic inflammatory response syndrome (SIRS). Auf der anderen Seite gibt es ein Gegenstück zu SIRS, nämlich das sogenannte compensatory anti-inflammatory response syndrome (CARS). Auch ein Überschießen von CARS kann den Verlauf der posttraumatischen Inflammation negativ beeinträchtigen. Sowohl eine Verschiebung in Richtung von SIRS als auch in Richtung von CARS kann zu Organfunktionsstörungen, nosokomialen Infektionen und letztendlich zum Tod führen. Daher ist eine ausgeglichene, frühe posttraumatische Immunantwort für ein gutes Outcome von entscheidender Bedeutung.
Monozyten nehmen eine kritische Stellung sowohl in der primären Immunantwort nach Trauma als auch nach Infektion ein. Durch die Oberflächenexpression von diversen sogenannten pattern-recocnition receptors (PRRs), insbesondere Toll-Like-Rezeptoren (TLRs), können Monozyten Pathogene und sogenannte pathogen-associated molecular patterns (PAMPs) erkennen und neutralisieren. Darüber hinaus können Monozyten PAMPs über major histocompatibility complex class II -Moleküle (MHC-II) dem adaptiven Immunsystem präsentieren und somit als zelluläre Verbindung zwischen dem angeborenen und dem adaptiven Immunsystem fungieren. TLR2, eine Untergruppe der TLRs, ist einer der Hauptrezeptoren für PAMPs von grampositiven Bakterien wie S. aureus, dem Hauptkeim für posttraumatische Wundinfektionen und nosokomiale Infektionen.
Es gibt immer noch keinen Konsens über den Einfluss von Trauma auf die Funktion von Monozyten und deren Expressionsprofil von PRRs und MHC-II-Molekülen nach einem Trauma. Verschiedene Studien berichten von einer beeinträchtigten TLR2-Expression bei Monozyten nach Trauma, während sich in anderen Studien eine konstante oder sogar erhöhte TLR2-Expression bei Monozyten nach Trauma gezeigt hat. Die Daten sind auch in Bezug auf die posttraumatische Phagozytoseleistung von Monozyten unbeständig. Während in einigen Studien im frühen posttraumatischen Verlauf im Vergleich zu gesunden Probanden von einer erhöhten Anzahl zirkulierender Monozyten mit erhaltener Phagozytoseleistung berichtet wird, zeigten andere Studien eine verminderte Phagozytoseleistung von Monozyten nach Trauma. Darüber hinaus gibt es widersprüchliche Ergebnisse bezüglich der Fähigkeit von Monozyten, proinflammatorische Zytokine wie Interleukin (IL)-1β oder Tumor necrosis factor (TNF)-α freizusetzen.
Zusammengefasst ist die derzeitige Studienlage widersprüchlich. Zusätzlich wurde es in bisherigen Studien verpasst, die Kombination verschiedener Aspekte wie die Funktionalität der Monozyten in Bezug auf Phänotypisierung oder funktioneller Assays und deren Beobachtung über einen längeren Zeitraum miteinzubeziehen.
Die unterschiedlichen Ergebnisse der bisherigen Studien könnte durch die Natur des Polytraumas bedingt sein, beispielsweise durch verschiedene Verletzungsmuster und Verletzungsschwere, die unterschiedliche Erstversorgung am Unfallort, die Zeit bis zur Ankunft in der Notaufnahme oder die unterschiedliche primäre und definitive Versorgung der Verletzungen. All das kann zu einer unterschiedlichen Antwort des Immunsystems und somit zu unterschiedlichen Outcomes führen.
Daher war es Ziel unserer Studie, die oben genannten Variablen zu eliminieren, und eine kontrollierte Polytraumastudie am Schweinemodell durchzuführen, um die Funktionalität von Schweinemonozyten über einen Zeitraum von 72 Stunden nach Trauma zu untersuchen.
Zusätzlich und im Gegensatz zu früheren Studien haben wir die Phänotypisierung von Monozyten (TLR2 und MHC-II [SLA-DR]) mit einem funktionellen Phagozytose-Assay kombiniert und deren direkte Assoziation in einem unabhängigen Assay analysiert.
Peripheres Blut wurde vor (-1h) und direkt nach der Induktion des Polytraumas (PT) (0h) entnommen, sowie 3,5h, 5,5h, 24h und 72h später. Die Expression von H(S)LA-DR und TLR2 auf Schweine-Monozyten wurde untersucht. Außerdem wurde die Phagozytierungsaktivität von Schweinemonozyten gemessen.
Darüber hinaus wurden aus mechanistischen Gründen Blutproben von 10 gesunden Schweinen zunächst einem TLR2-neutralisierenden Antikörper und anschließend S. aureus-Partikeln ausgesetzt, bevor die Phagozytoseleistung der Monozyten untersucht wurde.
Die Anzahl der CD14 + -Monozyten aller zirkulierenden Leukozyten blieb während des Beobachtungszeitraums konstant, während der Prozentsatz der CD14 + H(S)LA-DR + -Monozyten direkt, 3,5h und 5,5h nach dem Trauma signifikant abnahm. Der Prozentsatz von TLR2 + exprimierenden Zellen aus allen Monozyten verringerte sich direkt, 3,5h und 5,5h nach dem Trauma signifikant. Der Prozentsatz der phagozytierenden Monozyten nahm direkt nach Trauma ab und blieb in den ersten 3,5 Stunden nach dem Trauma niedriger, stieg jedoch nach 24 Stunden an. Die Antagonisierung von TLR2 verringerte signifikant die Phagozytoseleistung der Monozyten.
Sowohl der verringerte Prozentsatz der aktivierten als auch der TLR2-exprimierenden Monozyten blieb bestehen, solange die verringerte Phagozytoseleistung beobachtet wurde.
Darüber hinaus führte auch die Neutralisation von TLR2 zu einer verminderten Phagozytoseleistung. Daher nehmen wir an, dass eine verringerte TLR2-Expression für die verringerte Phagozytoseleistung verantwortlich ist.
Background: Developmentally adapted cognitive processing therapy (D-CPT) is an effective treatment for posttraumatic stress disorder (PTSD) in adolescents and young adults. It is unclear if therapeutic adherence and competence in D-CPT are associated with higher PTSD treatment gains.
Objective: To assess if higher therapeutic adherence and competence in D-CPT are associated with higher symptom reduction of PTSD in adolescents and young adults, while controlling for therapeutic alliance.
Participants and setting: Participants were 38 patients (aged 14–21 years; M = 17.61 years, SD = 2.42 years) of a multicenter randomized controlled trial in which the efficacy of D-CPT was compared to a waitlist with treatment advice.
Methods: Videotaped therapy sessions were rated using validated ratings scales to assess adherence and competence. Therapeutic alliance was assessed via weekly patient ratings. We used hierarchical linear modelling to assess the relationship of adherence and competence on PTSD symptoms being measured by both clinician and patient while controlling for alliance.
Results: Neither adherence nor competence were related to treatment outcomes in clinician or patient rated PTSD symptom severity. Higher alliance was associated with a lower symptom severity at 12 months posttreatment in both clinician and patient rated PTSD symptoms.
Conclusions: In this study of young adults with PTSD, who were treated with D-CPT by well-trained therapists, therapeutic adherence and competence were not related to treatment outcome. This might be explained by a lack of range in therapist adherence and competence. Therapeutic alliance had a positive effect on PTSD symptom severity.
The role of lncRNAs in the CVS and the endothelium is highly diverse and has been subject to a substantial amount of research over the last decade. The identification of lncRNAs as clinically relevant biomarkers and as co-regulatory molecules let to the appreciation of the functional relevance of lncRNAs.
In the present study, LINC00607 was identified as an endothelial-enriched, human-specific lncRNA. With its distinct functions, LINC00607 maintains and supports the endothelial homeostasis especially in response to VEGF-A signalling.
In the first part of this study, LINC00607 was functionally characterized in human endothelial cells. LINC00607 is highly and specifically expressed in endothelial cells and is differentially regulated in CVDs. Depletion of LINC00607 resulted in decreased angiogenic sprouting, reduced integration of ECs in a newly formed vascular network in vivo, enhanced endothelial migration and differential expression of many important genes for endothelial cell homeostasis. Functionally, LINC00607 maintains ERG-driven endothelial gene expression programs through BRG1. BRG1 secures stably accessible enhancer regions as well as TSS of ERG target genes, thus enabling transcription of endothelial gene programs.
The second part of this study proposes an additional mode of action for LINC00607. The strongly impaired response to VEGF-A after LINC00607 KO can only be partially explained by its’ expression control of ERG target genes. It rather appears that LINC00607 is involved in the control of alternative splicing of VEGF receptor FLT1. The differential splicing of FLT1 produces the anti-angiogenic soluble isoform of FLT1. Even though further validation is needed to uncover the underlying mechanism, there is the potential of a more general role of LINC00607 in splicing control through BRG1. As AS of FLT1 is a clinical marker in preeclampsia, LINC00607 might qualify to be an additional marker for the onset and manifestation of the pregnancy disorder.
Taken together, LINC00607 is a target in future for molecular therapy in CVD to restore a healthy endothelial phenotype and has the potential to serve as a biomarker in preeclampsia.
Behavioral and psychological syndromes such as depression and psychosis often occur along with cognitive (esp. executive) deficits in vascular cognitive disorder (VCD) in the elderly. We present the case of an 85-year-old woman with deficits in executive functions as well as a persistent and clearly circumscribed paranoid hallucinatory syndrome (most probably due to VCD) which could not be adequately treated with antipsychotic medication. The patient also suffered from severe depression (independent of psychotic symptoms). Both psychosis and depression were successfully managed in a home treatment based on Flexible Assertive Community Treatment (FACT). Interestingly, a thematic association between the delusional contents and early childhood traumata could be reconstructed, and late-onset trauma-related symptoms could be successfully treated with cognitive-behavioral therapy (CBT) as well. In sum, behavioral management of psychotic syndromes is possible in the absence of adequate pharmacological treatment options, and multiprofessional and person-centered home treatment may be successful in the elderly, even in severe and complex disorders.
Febrile neutropenia is a common infectious complication in children and adolescents receiving chemotherapy for cancer, requiring immediate hospitalisation and empirical antibacterial therapy. The risk for a severe infection increases with lower neutrophil counts, but other factors such as underlying malignancy, remission state or the genetic background might also impact on the risk and severity of infection. Initial antibacterial treatment as well as modification and cessation of therapy depends on clinical performance, microbiological findings and haematological recovery. Although paediatric specific guidelines have been developed in the last decade, a number of questions are still unsolved. This article gives an overview on diagnostics and management of paediatric patients presenting with febrile neutropenia, on research gaps and will speculate on future perspective.
In this comprehensive review, we will dissect the impact of research on proteoglycans focusing on recent developments involved in their synthesis, degradation, and interactions, while critically assessing their usefulness in various biological processes. The emerging roles of proteoglycans in global infections, specifically the SARS-CoV-2 pandemic, and their rising functions in regenerative medicine and biomaterial science have significantly affected our current view of proteoglycans and related compounds. The roles of proteoglycans in cancer biology and their potential use as a next-generation protein-based adjuvant therapy to combat cancer is also emerging as a constructive and potentially beneficial therapeutic strategy. We will discuss the role of proteoglycans in selected and emerging areas of proteoglycan science, such as neurodegenerative diseases, autophagy, angiogenesis, cancer, infections and their impact on mammalian diseases.
A 24-year-old patient from Cameroon presented to our hospital because of a foreign structure in her left eye. To our knowledge, for the first time, fluorescent microscopy revealed motile microfilariae, and the diagnosis of loiasis was established. Despite substantial microfilaremia, eosinophilia only unmasked after the initiation of antiparasitic therapy.

Abstract
Objectives: Patients with open pulmonary tuberculosis (opTB) are subject to strict isolation rules. Sputum smear microscopy is used to determine infectivity, but sensitivity is lower than for culture. This study aimed to investigate the clinical relevance of this mismatch in contemporary settings.
Methods: Differential results between microscopy and culture were determined at the time of microscopic sputum conversion, from all patients with opTB between 01/2013 and 12/2017. In addition, data on HIV, multi/extensive drug-resistant TB status, time to smear- and cultural-negativity conversion were analyzed; and a Kaplan-Meier curve was developed.
Results: Of 118 patients with opTB, 58 had demographic data available for microbiological and clinical follow-up analysis; among these, 26 (44.8%) had still at least one positive culture result. Median time from opTB-treatment initiation to full microscopic sputum- or culture conversion, was 16.5 days (range 2-105), and 20 days (1-105), respectively (median difference: +3.5 days). Sixteen days after de-isolation, >90% had converted culturally. HIV- or multi/extensive drug-resistant TB status did not impact conversion time.
Conclusion: When patients with opTB were de-isolated after 3 negative sputum smear microscopy tests, a substantial part still revealed cultural growth of Mycobacterium tuberculosis complex, but it remains unclear, whether smear-negative and culturally-positive individuals on therapy are really infective. Thus, the clinical relevance of this finding warrants further investigation.
Abnormal venous atrial (VA) connections present a congenital heart disease (CHD) challenge for pediatric cardiologists. Fully anatomical evaluation is very difficult in prenatal and perinatal follow-up, but it has a profound impact on surgical correction and outcome. The echocardiogram is first-line imaging and represents the gold standard tool for simple abnormal VA connection. CT and MRI are mandatory for more complex heart disease and “nightmare cases”. 3D post-processing of volumetric CT and MRI acquisition helps to clarify anatomical relationships and allows for the creation of 3D printing models that can become crucial in customizing surgical strategy.
Approximately 80 % of persistent wound infections are affected by the presence of bacterial biofilms, resulting in a severe clinical challenge associated with prolonged healing periods, increased morbidity, and high healthcare costs. Unfortunately, in vitro models for wound infection research almost exclusively focus on early infection stages with planktonic bacteria. In this study, we present a new approach to emulate biofilm-infected human wounds by three-dimensional human in vitro systems. For this purpose, a matured biofilm consisting of the clinical key wound pathogen Pseudomonas aeruginosa was pre-cultivated on electrospun scaffolds allowing for non-destructive transfer of the matured biofilm to human in vitro wound models. We infected tissue-engineered human in vitro skin models as well as ex vivo human skin explants with the biofilm and analyzed structural tissue characteristics, biofilm growth behavior, and biofilm-tissue interactions. The structural development of biofilms in close proximity to the tissue, resulting in high bacterial burden and in vivo-like morphology, confirmed a manifest wound infection on all tested wound models, validating their applicability for general investigations of biofilm growth and structure. The extent of bacterial colonization of the wound bed, as well as the subsequent changes in molecular composition of skin tissue, were inherently linked to the characteristics of the underlying wound models including their viability and origin. Notably, the immune response observed in viable ex vivo and in vitro models was consistent with previous in vivo reports. While ex vivo models offered greater complexity and closer similarity to the in vivo conditions, in vitro models consistently demonstrated higher reproducibility. As a consequence, when focusing on direct biofilm-skin interactions, the viability of the wound models as well as their advantages and limitations should be aligned to the particular research question of future studies. Altogether, the novel model allows for a systematic investigation of host-pathogen interactions of bacterial biofilms and human wound tissue, also paving the way for development and predictive testing of novel therapeutics to combat biofilm-infected wounds.
For medicine to fulfill its promise of personalized treatments based on a better understanding of disease biology, computational and statistical tools must exist to analyze the increasing amount of patient data that becomes available. A particular challenge is that several types of data are being measured to cope with the complexity of the underlying systems, enhance predictive modeling and enrich molecular understanding.
Here we review a number of recent approaches that specialize in the analysis of multimodal data in the context of predictive biomedicine. We focus on methods that combine different OMIC measurements with image or genome variation data. Our overview shows the diversity of methods that address analysis challenges and reveals new avenues for novel developments.
Mitochondrial matrix peptidase CLPP is crucial during cell stress. Its loss causes Perrault syndrome type 3 (PRLTS3) with infertility, neurodegeneration and growth deficit. Its target proteins are disaggregated by CLPX, which also regulates heme biosynthesis via unfolding ALAS enzyme, providing access of pyridoxal-5’-phosphate (PLP). Despite efforts in diverse organisms with multiple techniques, CLPXP substrates remain controversial. Here, avoiding recombinant overexpression, we employed complexomics in mitochondria from three mouse tissues to identify endogenous targets. CLPP absence caused accumulation and dispersion of CLPX-VWA8 as AAA+ unfoldases, and of PLPBP. Similar changes and CLPX-VWA8 comigration were evident for mitoribosomal central protuberance clusters, translation factors like GFM1-HARS2, RNA granule components LRPPRC-SLIRP, and enzymes OAT-ALDH18A1. Mitochondrially translated proteins in testis showed reductions to <30% for MTCO1-3, misassembly of complex-IV supercomplex, and accumulated metal-binding assembly factors COX15-SFXN4. Indeed, heavy metal levels were increased for iron, molybdenum, cobalt and manganese. RT-qPCR showed compensatory downregulation only for Clpx mRNA, most accumulated proteins appeared transcriptionally upregulated. Immunoblots validated VWA8, MRPL38, MRPL18, GFM1 and OAT accumulation. Coimmunoprecipitation confirmed CLPX binding to MRPL38, GFM1 and OAT, so excess CLPX and PLP may affect their activity. Our data elucidate mechanistically the mitochondrial translation fidelity deficits, which underlie progressive hearing impairment in PRLTS3.

A distinguishing feature of retroviruses is reverse transcription, where the +ssRNA genome serves as a template to synthesize a double-stranded DNA copy that subsequently integrates into the host genome. As retroviral RNAs are produced by the host cell transcriptional machinery and are largely indistinguishable from cellular mRNAs, we investigated the potential of incoming retroviral genomes to directly express proteins. Here we show through multiple, complementary methods that retroviral genomes are translated after entry. Our findings challenge the notion that retroviruses require reverse transcription to produce viral proteins. Synthesis of retroviral proteins in the absence of productive infection has significant implications for basic retrovirology, immune responses and gene therapy applications.

Human feline leukemia virus subgroup C receptor-related proteins 1 and 2 (FLVCR1 and 2) are members of the major facilitator superfamily1. Their dysfunction is linked to several clinical disorders, including PCARP, HSAN, and Fowler syndrome2–7. Earlier studies concluded that FLVCR1 may function as a putative heme exporter8–12, while FLVCR2 was suggested to act as a heme importer13, yet conclusive biochemical and detailed molecular evidence remained elusive for the function of both transporters14–17. Here, we show that FLVCR1 and FLVCR2 facilitate the transport of choline and ethanolamine across human plasma membranes, utilizing a concentration-driven substrate translocation process. Through structural and computational analyses, we have identified distinct conformational states of FLVCRs and unraveled the coordination chemistry underlying their substrate interactions. Within the binding pocket of both transporters, we identify fully conserved tryptophan and tyrosine residues holding a central role in the formation of cation-π interactions, essential for choline and ethanolamine selectivity. Our findings not only clarify the mechanisms of choline and ethanolamine transport by FLVCR1 and FLVCR2, enhancing our comprehension of disease-associated mutations that interfere with these vital processes, but also shed light on the conformational dynamics of these MFS-type proteins during the transport cycle.
Zielsetzung der vorliegenden Dissertation war die Validierung einer deutschsprachigen Version, der bereits für andere Sprachen validierten Repetitiven Verhaltensskala – Revidiert (RBS-R) für Kinder- und Jugendliche im Alter zwischen 4-17 Jahren. Die RBS-R ist ein aus 43 Items bestehendes, gut untersuchtes Screeninginstrument, welches restriktives, repetitives Verhalten (RRV) erfasst. RRV stellen einen heterogenen Symptomkomplex dar, welcher durch stereotype motorische, sprachliche und kognitive Verhaltensmuster gekennzeichnet ist.
RRV sind neben Abweichungen sozialer Interaktion und Kommunikation eines der Hauptkriterien für eine Autismus-Spektrum-Störung (ASS). RRV sind jedoch nicht nur Teil des Symptomspektrums einer ASS, sondern treten häufig auch bei anderen psychiatrischen Erkrankungen (PE), einer Intelligenzminderung (IM) oder bei altersentsprechend entwickelten Kindern (AE), beispielsweise in bestimmten Phasen des Heranwachsens auf. Bisher wurde die RBS-R fast ausschließlich an ASS untersucht. Es besteht die Notwendigkeit für ein valides und zuverlässiges Messinstrument für den deutschen Sprachraum, welches RRV hinsichtlich ihrer Heterogenität transdiagnostisch erfassen und den Schweregrad der Ausprägung ermitteln kann. Das Ziel dieser Arbeit war, dies durch eine möglichst große und heterogene Stichprobe (n= 948) mit Inklusion von Probanden mit einer ASS (n= 218), IM (n= 120), PE (n= 166) und AE (n = 444) zu gewährleisten. Zur Überprüfung der psychometrischen Eignung erfolgte nach der Adaptation der RBS-R für den deutschen Sprachraum eine Evaluation gemäß den Prinzipien der klassischen Testtheorie, insbesondere hinsichtlich Validität und Reliabilität. Darüber hinaus erfolgte unter Einbeziehung der vier Stichproben eine explorative Faktoranalyse (EFA) um eine geeignete Faktorlösung zu finden. Diese sollte sowohl den Anspruch Klinisch-Tätiger als auch Forschender erfüllen und eine Anwendung der RBS-R auch außerhalb von ASS ermöglichen. Zudem sollten RRV in den unterschiedlichen Kohorten, sowie in Bezug auf Alters- und Geschlechtseffekte bei ASS untersucht werden.
Die Bedeutung der Ergebnisse dieser Arbeit ist, ob der schon breit erforschten Eigenschaften der RBS-R, vor allem im Kontext mit den vorliegenden Studien zu betrachten. Einzigartig für diese deutschsprachige Validierung der RBS-R ist die Diversität und große Anzahl der teilnehmenden Probanden.
Die vorliegende Validierung der RBS-R erbrachte gute Ergebnisse für die Reliabilität und konvergente Validität der Skalen, vergleichbar mit den Ergebnissen vorheriger Studien. Die untersuchte Itemschwierigkeit war relativ gering, was durch die geringe Antwortvarianz in der großen Kohorte der AE erklärbar ist. Die Itemgesamtkorrelation zeigte gute Werte. Auch Items mit niedrigen Ergebnissen für die Itemzustimmung wurden nicht exkludiert. Dies geschah konkordant zum Vorgehen vorheriger Studien und hatte den Hintergrund, eine internationale Vergleichbarkeit des RBS-R beizubehalten. Die EFA bestätigte die gute Anwendbarkeit einer Vielzahl von Faktorlösungen. Wir legten eine 4-Faktorlösung für die deutschsprachige Version der RBS-R fest. Grundlage hierfür war das Bestreben die ursprünglichen Subskalen für eine internationale Vergleichbarkeit so getreu wie möglich beizubehalten. Die 4 Faktoren bildeten treffend RRV niedriger und hoher Ordnung ab. Gegen eine 5- oder 6-Faktorlösung sprach hierbei, dass dies zu Subskalen mit wenig Items und schwachen psychometrischen Eigenschaften führte (Lam et al, kritisiert durch Georgiades et al). Gegen eine 2- oder 3-Faktorlösung sprach die eingeschränkte klinische Interpretierbarkeit durch Subskalen mit vielen kumulierten Items. Hinsichtlich der Kohortenzugehörigkeit kann der RBS-R übereinstimmend mit Fulceri et al valide zwischen RRV in ASS und AE unterscheiden. Eine valide Unterscheidung zwischen ASS, PE und IM konnten wir nicht verifizieren. Betreffend der untersuchten Alterseffekts bestätigte sich, dass RRV bei ASS mit dem Alter abnehmen. Im Gegensatz zu einigen vorherigen Studien, welche keinen Geschlechtsunterschied bezüglich RRV bei ASS fanden, zeigten die männlichen Probanden der ASS Kohorte bei uns häufiger und ausgeprägter RRV.
