Apoptose ist ein gemeinsamer Pathomechanismus vieler Autoimmunerkrankungen, so auch im Rahmen der Schilddrüsenautoimmunerkrankungen, deren häufigste Hashimoto Thyreoiditis und Morbus Basedow (Graves’ disease) darstellen. Während eine gesteigerte Apoptoserate der Schilddrüsenzellen bei Hashimoto Thyreoiditis zur Entwicklung einer Hypothyreose führt, sind es bei Morbus Basedow die infiltrierenden Lymphozyten, die durch Apoptose zu Grunde gehen, während die Schilddrüsenzellen unter dem protektiven Einfluß anti-apoptotischer Signale proliferieren können. Der zu Grunde liegende Pathomechanismus wird in der Reifung und Aktivierung des Immunsystems angenommen. Einige potentielle Ursachen sind eine insuffiziente systemische Elimination autoreaktiver Lymphozyten im Rahmen der positiven und negativen Selektion im Thymus, ein Versagen der peripheren immunological ignorance, oder eine inadäquate Aktivierung von Lymphozyten in einem pro-inflammatorischen Milieu. All diese Mechanismen können durch eine inadäquate Apoptose ausgelöst oder verstärkt werden. Die Fas/FasL-induzierte Kaskade ist einer der Haupt-Signalwege in der Apoptose-Induktion. Seine Bedeutung im Rahmen von Autoimmunerkrankungen ist vielfach untersucht, und eine Assoziation genetischer Polymorphismen in Fas und seinem Liganden wurde bereits für einige organspezifische und generalisierte Erkrankungen gezeigt (Typ I Diabetes, Multiple Sklerose, Rheumatoide Arthritis u.a.). Das genetisch vermittelte Risiko dieser Erkrankungen wird bisher vor allem der HLA DQ-Region und Polymorphismen im CTLA-4-Gen zugeschrieben. Aufgrund der funktionellen Relevanz der Fas/FasL-vermittelten Apoptose im Rahmen autoimmuner Schilddrüsenerkrankungen wurden im Rahmen dieser Arbeit Kandidatengene untersucht, die von funktioneller Relevanz für die Entstehung autoimmuner Schilddrüsenerkrankungen sind: die Apoptose-regulierenden Faktoren Fas, FasL und Bcl-2. 209 Familien kaukasischen Ursprungs mit insgesamt 730 Individuen, bei denen mindestens eines der Kinder an Hashimoto Thyreoiditis (n = 100) oder an Morbus Basedow (n = 109) erkrankt war, wurden genotypisiert auf zwei Polymorphismen des Fas-Gens (g-670 G-A in der Promotorregion, g-154 C-T in Exon 7), drei FasL Genpolymorphismen (C -843 T und EBP im Promotor, A IVS2nt-124 G in Intron 2) und einen Polymorphismus im Bcl-2-Gen. Nach Durchführung einer indirekten Haplotypisierung, Chi2- und Extended Transmission Disequilibrium-Test waren weder einzelne Fas- bzw. FasL-Polymorphismen noch kombinierte Haplotypen mit Hashimoto Thyreoiditis oder Morbus Basedow assoziiert. Bei der Analyse des Bcl-2 Polymorphismus ergab sich eine Tendenz zu einer Assoziation des G-Allels mit beiden Thyreopathien. Subgruppen-Analysen (männliche vs. weibliche Patienten, CTLA-4-Genotypen, DQ-Haplotypen) ergaben keinen weiteren Informationsgehalt, die Daten italienischer und deutscher Familien unterschieden sich ebenfalls nicht. Zusammenfassend ergeben die Daten keinen signifikanten Beitrag der untersuchten Varianten in Bcl-2, Fas oder FasL zum genetischen Risiko autoimmuner Schilddrüsenerkrankungen. Zur Entwicklung möglicher diagnostischer und therapeutischer Optionen für autoimmune Schilddrüsenerkrankungen wird die Untersuchung weiterer funktionell bedeutsamer Regulatoren der apoptotischen Signaltransduktionswege von Bedeutung sein.
Purpose: The aim of the study was to compare three different elastography methods, namely Strain Elastography (SE), Point Shear-Wave Elastography (pSWE) using Acoustic Radiation Force Impulse (ARFI)-Imaging and 2D-Shear Wave Elastography (2D-SWE), in the same study population for the differentiation of thyroid nodules.
Materials and methods: All patients received a conventional ultrasound scan, SE and 2D-SWE, and all patients except for two received ARFI-Imaging. Cytology/histology of thyroid nodules was used as a reference method. SE measures the relative stiffness within the region of interest (ROI) using the surrounding tissue as reference tissue. ARFI mechanically excites the tissue at the ROI using acoustic pulses to generate localized tissue displacements. 2D-SWE measures tissue elasticity using the velocity of many shear waves as they propagate through the tissue.
Results: 84 nodules (73 benign and 11 malignant) in 62 patients were analyzed. Sensitivity, specificity and NPV of SE were 73%, 70% and 94%, respectively. Sensitivity, specificity and NPV of ARFI and 2D-SWE were 90%, 79%, 98% and 73%, 67%, 94% respectively, using a cut-off value of 1.98m/s for ARFI and 2.65m/s (21.07kPa) for 2D-SWE. The AUROC (Area under the Receiver Operating Characteristic) of SE, ARFI and 2D-SWE for the diagnosis of malignant thyroid nodules were 52%, 86% and 71%, respectively. A significant difference in AUROC was found between SE and ARFI (p = 0.008), while no significant difference was found between ARFI and SWE (86% vs. 71%, p = 0.31), or SWE and SE (71% vs. 52%, p = 0.26).
Conclusion: pSWE using ARFI and 2D-SWE showed comparable results for the differentiation of thyroid nodules. ARFI was superior to elastography using SE.
Aim: Radioiodine therapy (RIT) may trigger the development of Graves’ ophthalmopathy (GO) or exacerbate pre-existing subclinical GO. Therefore, glucocorticoid administration is recommended for patients with pre-existing GO. Aim of this study was to analyze the influence of glucocorticoid therapy with methylprednisolone on intratherapeutic effective half-life (EHL) of radioiodine-131 in patients with Graves’ disease (GD) as recent studies showed an effect for prednisolone.
Methods: In a retrospective study, 264 patients with GD who underwent RIT without any additional antithyroid medication were evaluated. Intrathyroidal EHL was determined pre- and intratherapeutically. Patients with co-existing GO (n = 43) received methylprednisolone according to a fixed scheme starting 1 day prior to RIT, patients without GO (n = 221) did not receive any protective glucocorticoid medication. The ratios of EHL during RIT and during radioiodine uptake test (RIUT) were compared.
Results: Patients receiving methylprednisolone showed a slight decrease of the mean EHL from 5.63 d (RIUT) to 5.39 d (RIT) (p > 0.05). A comparable result was obtained in patients without glucocorticoids (5.71 d (RIUT) to 5.47 d (RIT); p > 0.05). The ratios of the EHL between RIT and RIUT failed to show a significant difference between the two groups. EHL is therefore not significantly influenced by an additional protective treatment with methylprednisolone.
Conclusions: In the present study a decreased intrathyroidal EHL under glucocorticoid medication with methylprednisolone could not be detected. Therefore, co-medication with methylprednisolone in patients with GO may be preferred to avoid an intratherapeutic decrease of EHL by accompanying protective glucocorticoides.
