Immune profile and radiological characteristics of progressive multifocal leukoencephalopathy

Background and purpose: Progressive multifocal leukoencephalopathy (PML) constitutes a severe disease with increasing incidence, mostly in the context of immunosuppressive therapies. A detailed understanding of immune response in PML appears critical for the treatment strategy. The aim was a comprehensive immunoprofiling and radiological characterization of magnetic resonance imaging (MRI) defined PML variants.
Methods: All biopsy-confirmed PML patients (n = 15) treated in our department between January 2004 and July 2019 were retrospectively analysed. Data from MRI, histology as well as detailed clinical and outcome data were collected. The MRI-defined variants of classical (cPML) and inflammatory (iPML) PML were discriminated based on the intensity of gadolinium enhancement. In these PML variants, intensity and localization (perivascular vs. parenchymal) of inflammation in MRI and histology as well as the cellular composition by immunohistochemistry were assessed. The size of the demyelinating lesions was correlated with immune cell infiltration.
Results: Patients with MRI-defined iPML showed a stronger intensity of inflammation with an increased lymphocyte infiltration on histological level. Also, iPML was characterized by a predominantly perivascular inflammation. However, cPML patients also demonstrated certain inflammatory tissue alterations. Infiltration of CD163-positive microglia and macrophage (M/M) subtypes correlated with PML lesion size.
Conclusions: The non-invasive MRI-based discrimination of PML variants allows for an estimation of inflammatory tissue alterations, although exhibiting limitations in MRI-defined cPML. The association of a distinct phagocytic M/M subtype with the extent of demyelination might reflect disease progression.
Background: The fascia has been demonstrated to represent a potential force transmitter intimately connected to the underlying skeletal muscle. Sports-related soft tissue strains may therefore result in damage to both structures.
Purpose: To elucidate the prevalence of connective tissue lesions in muscle strain injury and their potential impact on return-to-play (RTP) duration.
Study Design: Systematic review; Level of evidence, 3.
Methods: Imaging studies describing frequency, location, and extent of soft tissue lesions in lower limb muscle strain injuries were identified by 2 independent investigators. Weighted proportions (random effects) were pooled for the occurrence of (1) myofascial or fascial lesions, (2) myotendinous lesions, and (3) purely muscular lesions. Study quality was evaluated by means of an adapted Downs and Black checklist, which evaluates reporting, risk of bias, and external validity.
Results: A total of 16 studies (fair to good methodological quality) were identified. Prevalence of strain injury on imaging studies was 32.1% (95% CI, 24.2%-40.4%) for myofascial lesions, 68.4% (95% CI, 59.6%-76.6%) for myotendinous lesions, and 12.7% (95% CI, 3.0%-27.7%) for isolated muscular lesions. Evidence regarding associations between fascial damage and RTP duration was mixed.
Conclusion: Lesions of the collagenous connective tissue, namely the fascia and the tendinous junction, are highly prevalent in athletic muscle strain injuries. However, at present, their impact on RTP duration is unclear and requires further investigation.
The purpose of this phase III clinical trial was to compare two different extracellular contrast agents, 1.0 M gadobutrol and 0.5 M gadopentate dimeglumine, for magnetic resonance imaging (MRI) in patients with known or suspected focal renal lesions. Using a multicenter, single-blind, interindividual, randomized study design, both contrast agents were compared in a total of 471 patients regarding their diagnostic accuracy, sensitivity, and specificity to correctly classify focal lesions of the kidney. To test for noninferiority the diagnostic accuracy rates for both contrast agents were compared with CT results based on a blinded reading. The average diagnostic accuracy across the three blinded readers (‘average reader’) was 83.7% for gadobutrol and 87.3% for gadopentate dimeglumine. The increase in accuracy from precontrast to combined precontrast and postcontrast MRI was 8.0% for gadobutrol and 6.9% for gadopentate dimeglumine. Sensitivity of the average reader was 85.2% for gadobutrol and 88.7% for gadopentate dimeglumine. Specificity of the average reader was 82.1% for gadobutrol and 86.1% for gadopentate dimeglumine. In conclusion, this study documents evidence for the noninferiority of a single i.v. bolus injection of 1.0 M gadobutrol compared with 0.5 M gadopentate dimeglumine in the diagnostic assessment of renal lesions with CE-MRI.
Background: Hypoxia is a key driver for infiltrative growth in experimental gliomas. It has remained elusive whether tumor hypoxia in glioblastoma patients contributes to distant or diffuse recurrences. We therefore investigated the influence of perioperative cerebral ischemia on patterns of progression in glioblastoma patients.
Methods: We retrospectively screened MRI scans of 245 patients with newly diagnosed glioblastoma undergoing resection for perioperative ischemia near the resection cavity. 46 showed relevant ischemia nearby the resection cavity. A control cohort without perioperative ischemia was generated by a 1:1 matching using an algorithm based on gender, age and adjuvant treatment. Both cohorts were analyzed for patterns of progression by a blinded neuroradiologist.
Results: The percentage of diffuse or distant recurrences at first relapse was significantly higher in the cohort with perioperative ischemia (61.1%) compared to the control cohort (19.4%). The results of the control cohort matched well with historical data. The change in patterns of progression was not associated with a difference in survival.
Conclusions: This study reveals an unrecognized association of perioperative cerebral ischemia with distant or diffuse recurrence in glioblastoma. It is the first clinical study supporting the concept that hypoxia is a key driver of infiltrative tumor growth in glioblastoma patients.
Die Multiple Sklerose (MS) gehört zu den häufigsten chronisch-entzündlichen Erkrankungen des zentralen Nervensystems in Deutschland und kann durch Sehstörungen, Paresen oder Sensibilitätsstörungen symptomatisch werden.
Konventionelle Magnetresonanztomographie (MRT)-Verfahren leisten in der Diagnostik der MS einen wichtigen Beitrag, da diese die Läsionslast der weißen Substanz gut darstellen können. Frühere Studien deuten an, dass kognitive und psychomotorische Symptome wie Fatigue sowie Konzentrations- und Gedächtnisstörungen bei der MS mit Schädigungen des zerebralen Kortex in Beziehung stehen könnten. Mit konventionellen MRT-Bildgebungsverfahren lässt sich zwar kortikale Atrophie, nicht jedoch die zugrundeliegenden mikrostrukturellen kortikalen Umbauprozesse erfassen. In der vorliegenden Studie wurden daher quantitative MRT(qMRT)-Verfahren verwendet, die eben diese diffusen kortikalen Gewebsveränderungen messen und quantifizieren können. Mithilfe der dabei genutzten Diffusions-Tensor-Bildgebung (DTI) als qMRT-Verfahren konnten Diffusionsanomalien analysiert und charakterisiert werden. Dabei wurden zwei Gewebsparameter im Gehirn bestimmt: die mittlere Diffusivität(MD) und die fraktionelle Anisotropie (FA). Da vorherige Studien uneinheitliche Ergebnisse hinsichtlich Änderungen von DTI-Parametern in der grauen Substanz bei der MS erbrachten, beschäftigten wir uns mit der Frage, ob kortikale MD- und FA-Veränderungen bei Patienten mit schubförmig-remittierender MS (RRMS) mithilfe optimierter DTI-Messtechniken zu detektieren sind, wie diese charakterisiert sind und wie sich diese im Kortex verteilen.
An der vorliegenden Studie nahmen 24 Patienten mit RRMS und 25 gesunde Kontrollprobanden teil. Der Schweregrad der Erkrankung wurde mithilfe des Expanded Disability Status Scale (EDSS) eingestuft.
Bei der MRT-Datenerfassung wurde eine optimierte DTI-Methode mit intrinsischer „Eddy-Current“-Kompensation verwendet. Die MD und die FA wurden für jeden Bildpunkt bestimmt. Kortikale Parameterwerte wurden ausgelesen und in Oberflächendatensätzen gespeichert. Es erfolgte ein oberflächenbasierter statistischer Gruppenvergleich. Kortikale Mittelwerte wurden für die MD und die FA bestimmt und zwischen den Gruppen verglichen.
Für Parameter mit nachgewiesenen globalen Gruppenunterschieden wurde die Korrelation mit dem klinischen Status (quantifiziert durch den EDSS) bestimmt.
Die Analyse kortikaler Mittelwerte zeigte eine Erhöhung der MD in der Patientengruppe. Die MD-Veränderungen waren räumlich ausgedehnt und es fanden sich Cluster mit erhöhten MD-Werten in der Patientengruppe, insbesondere in temporalen, okzipitalen und parietalen Regionen. Des Weiteren konnte eine signifikante positive Korrelation zwischen dem EDSS-Score und der kortikalen MD festgestellt werden. Außerdem ließen sich fokale FA-Erniedrigungen im Temporal- und Okzipitallappen nachweisen. Die MD quantifiziert das Ausmaß und die FA die Gerichtetheit der Diffusion.
Somit bietet die MD möglicherweise Hinweise auf die Intaktheit mikrostruktureller Barrieren und die FA auf die Integrität von Faserverbindungen. Unsere Ergebnisse könnten demnach darauf hinweisen, dass im Kortex von MS-Patienten der Abbau mikrostruktureller Barrieren räumlich ausgedehnter stattfindet als eine Störung axonaler Strukturen. Die Korrelation der MD mit dem klinischen Status legt die Möglichkeit der Quantifizierung klinisch relevanter kortikaler Gewebsveränderungen und somit eine mögliche Relevanz dieser Techniken für klinische Studien nahe.
Einleitung: Die konventionelle Galaktografie stellte jahrzehntelang das einzige bildgebende Verfahren zur Darstellung von Milchgängen in der Brust dar. Heute verfügen wir in der Diagnostik über ein multimodales Konzept aus hochauflösendem Ultraschall, der Magnetresonanz-(MR-)Mammografie und der Duktoskopie/Galaktoskopie mit Sensitivitäten und Spezifitäten bis zu 95%. Ziel unserer Untersuchung war es, erstmalig die Tomosynthesetechnik in der Galaktografie einzusetzen und die daraus generierten synthetischen digitalen 2-D-Vollfeld-Mammografien mit der etablierten Methode der duktusorientierten Sonografie zu vergleichen. Es sollen mit beiden Methoden invasive Mammakarzinome und deren Vorstufen wie duktale Carcinoma in situ (DCIS) sowie benigne Befunde erkannt werden. Material und Methoden: Wir führten bei 5 Patientinnen mit pathologischer Mamillensekretion sowohl eine duktusorientierte Sonografie, eine kontrastmittelunterstützte Galaktografie mithilfe der Tomosynthese in 3-D sowie auch den daraus generierten synthetischen digitalen 2-D-Vollfeld-Mammografien durch. Die Auswertung der unterschiedlichen Untersuchungsmodalitäten erfolgte durch 3 in der komplementären Mammadiagnostik erfahrene Untersucher (1, 5 und 15 Jahre) und wurde mit der endgültigen Histologie korreliert. Ergebnisse: Alle 3 Untersucher beurteilten unabhängig voneinander die Bilder des duktusorientierten Ultraschalls und der kontrastmittelunterstützten Galaktografie in Tomosynthesetechnik in 3-D und den daraus generierten, synthetischen digitalen 2-D-Vollfeld-Mammografien. Die Ergebnisse wurden mit den histopathologischen Befunden der Operationspräparate korreliert, wobei sich bei den 5 Patientinnen 1 invasives Mammakarzinom, 2-mal ein duktales Carcinoma in situ (DCIS) und 2 benigne Befunde ergaben. Alle drei Untersucher lagen bei der Verdachtsdiagnose in der Standardbildgebung der duktusorientierten Sonografie seltener richtig als bei der erstmalig durchgeführten, kontrastmittelunterstützten Galaktografie in Tomosynthesetechnik und den daraus generierten, synthetischen digitalen 2-D-Vollfeld-Mammografien. Schlussfolgerung: Erstmalig wurde die Brusttomosynthese in der Galaktografie (Galaktomosynthese) eingesetzt und ermöglichte eine digitale, 3-dimensionale Darstellung von suspekten Befunden. Zusammen mit den daraus synthetisierten, digitalen 2-D-Vollfeld-Mammografien könnte dies in Zukunft eine sinnvolle Ergänzung der komplementären Mammadiagnostik sein – und eine Renaissance dieser Methode. Im Vergleich mit dem duktusorientierten Ultraschall in Hochauflösung erzielten die Untersucher mit der kontrastmittelunterstützten Galaktografie in Tomosynthesetechnik und den daraus generierten, synthetischen digitalen 2-D-Vollfeld-Mammografien bessere Ergebnisse in Korrelation mit den histopathologischen Befunden.
Introduction: For decades, conventional galactography was the only imaging technique capable of showing the mammary ducts. Today, diagnosis is based on a multimodal concept which combines high-resolution ultrasound with magnetic resonance (MR) mammography and ductoscopy/galactoscopy and has a sensitivity and specificity of up to 95%. This study used tomosynthesis in galactography for the first time and compared the synthetic digital 2D full-field mammograms generated with this technique with the images created using the established method of ductal sonography. Both methods should be able to detect invasive breast cancers and their precursors such as ductal carcinoma in situ (DCIS) as well as being able to identify benign findings.
Material and Methods: Five patients with pathological nipple discharge were examined using ductal sonography, contrast-enhanced 3D galactography with tomosynthesis and the synthetic digital 2D full-field mammograms generated with the latter method. Evaluation of the images created with the different imaging modalities was done by three investigators with varying levels of experience with complementary breast diagnostics (1, 5 and 15 years), and their evaluations were compared with the histological findings.
Results: All 3 investigators independently evaluated the images created with ductal sonography, contrast-enhanced 3D galactography with tomosynthesis, and generated synthetic digital 2D full-field mammograms. Their evaluations were compared with the histopathological assessment of the surgical specimens resected from the 5 patients. There was 1 case of invasive breast cancer, 2 cases with ductal carcinoma in situ and 2 cases with benign findings. All 3 investigators made more mistakes when they used the standard imaging technique of ductal sonography to diagnose suspicious lesions than when they used contrast-enhanced galactography with tomosynthesis and the generated synthetic digital 2D full-field mammograms.
Conclusion: This is the first time breast tomosynthesis was used in galactography (galactomosynthesis) to create digital 3-dimensional images of suspicious findings. When used together with the generated synthetic digital 2D full-field mammograms, it could be a useful complementary procedure for the diagnosis of breast anomalies and could herald a renaissance of this method. Compared with high-resolution ductal ultrasound, the investigators achieved better results with contrast-enhanced galactography using tomosynthesis and the generated synthetic digital 2D full-field mammograms, as confirmed by histopathological findings.
Das Ziel der vorliegenden Arbeit war es festzustellen (im Rahmen einer Doppelblindstudie mit 32 Patienten), ob der Einsatz des neu entwickelten Kontrastmittels Gd-EOB-DTPA in der MRT-Diagnostik im Vergleich zum herkömmlichen Gd-BOPTA gleichwertige Ergebnisse hervorbringt in Bezug auf Detektion und Charakterisierung von fokalen Leberläsionen und in der Gefäß- und Leberparenchymdarstellung. Gd-EOB-DTPA zeigte eine sehr gute Verträglichkeit, die sich in der vorliegenden Studie als gleichwertig zu Gd-BOPTA erwies. Einen Vorteil gegenüber Gd-BOPTA bietet Gd-EOB-DTPA durch eine frühere leberspezifische Phase; auf diese Weise lassen sich dynamische Sequenzen und hepatospezifische Untersuchungen in einem einzigen Untersuchungsdurchgang vornehmen. Insgesamt kann Gd-EOB-DTPA als hochwertiges, Gd-BOPTA gleichgestelltes hepatospezifisches MRT-Kontrastmittel betrachtet werden und ist sicherlich für den breiten klinischen Einsatz gut geeignet.
Rationale and objectives: The objective of this study was to analyze the role of dynamic magnetic resonance imaging (MRI) in patients who suffered from groin pain and whose physical examination and ultrasound returned inconclusive/indefinite results, as well as in patients receiving an ongoing assessment for a previous herniotomy.
Material and methods: For this study, 25 patients 14 women and 11 men were selected with a mean age of 41.6 years, including clinical complaints, such as groin pain and or a previous herniotomies. These patients underwent dynamic MRI. Reports were created by a radiology resident and a radiology consultant. Clinical and ultrasound documentation were compared to with imaging results from the MRI.
Results: The results of the dynamic MRI were negative for 23 patients (92%) and positive for two patients (8%). One patient suffered from an indirect hernia and one from a femoral hernia. A repeated hernia was an excluding for the preoperated patients with pain and ongoing assessment.
Conclusions: Dynamic MRI shows substantially higher diagnostic performance in exclusion of inguinal hernia, when compared to a physical examination and ultrasound. The examination can also be used in assessments to analyze the operation’s results.
Ziel der Arbeit war die Verifizierung der MR-Thermometrie mit verschiedenen MR Sequenzen für die laserinduzierte Thermotherapie mittels fluoroptischer Temperaturmessung bei 0,2 und 1,5 Tesla, bei Temperaturen bis 80 Grad Celsius. Bei ex-vivo Schweineleber und Agarose-Phantomen wurde unter MR-Bildgebung eine laserinduzierte Thermotherapie (LITT) durchgeführt. Die Messungen erfolgten pro Tomograph mit zwei verschiedenen Empfangsspulen. Die Temperaturdarstellung basierte auf der Änderung der Protonenresonanzfrequenz (PRF) und der longitudinalen Relaxationszeit (T1). Die PRF wurde mit vier verschiedenen MR-Sequenzen gemessen: zwei Gradientenecho-Sequenzen (FLASH), einer TurboFLASH- und einer Multiecho-TRUFI-Sequenz. Bei der T1-Methode wurden ebenfalls vier verschiedene MR-Sequenzen eingesetzt: eine konventionelle Gradientenecho-Sequenz (FLASH), eine TrueFISP-Sequenz (TRUFI), eine Saturation Recovery Turbo-FLASH-Sequenz (SRTF) und eine Inversion Recovery Turbo-FLASH-Sequenz (IRTF). Die Temperatur wurde mit einem faseroptischen Thermometer kontrolliert und mit der MRT-Temperatur korreliert. Es wurde eine gute lineare Korrelation zwischen der am MRT geschätzten und der faseroptisch gemessenen Temperatur erreicht. Bei 1,5 Tesla unter Einsatz einer Kopfspule erwies sich bei Messungen an der Schweineleberprobe PRF-FLASH von Siemens mit einer mittleren Temperaturabweichung von 5,09°C als optimal. Mit einem Bodyarray ergab hier IRTF die präzisesten Temperaturbestimmungen mit einer mittleren Abweichung von 8,02°C. Die Genauigkeiten und die Linearitäten von SRTF und PRF-TFL unterschieden sich davon nur geringfügig, sie können also als gleichwertig betrachtet werden. Bei 0,2 Tesla mit einer Kopfspule ergab die Messung mit SRTF mit 6,4°C die geringste mittlere Temperaturabweichung, mit einer Multipurpose-Coil erwiesen sich TRUFI und FLASH als optimal mit einer mittleren Temperaturabweichung von 15,62°C. bzw. 14,48°C. Mit den erreichten Temperaturgenauigkeiten kann der Thermoeffekt der LITT in Echtzeitnähe kontrolliert werden. Bei 1,5 T sind PRF-FLASH oder TFL aufgrund der Exaktheit und der Gewebeunabhängigkeit vorzuziehen. PRF-TRUFI mit einer Akquisitionszeit von 1,09 s ist die schnellste implementierte Sequenz. Bei 0,2 T sind die T1-Sequenzen genauer.
