Treatment predictors are important tools for the management of therapy in patients with chronic hepatitis B and C virus (HBV, HCV) infection. In chronic hepatitis B, several pretreatment parameters have been identified for prediction of virologic response to interferon alfa-based antiviral therapies or treatment with polymerase inhibitors. In interferon alfa and pegylated interferon alfa-treated patients, low baseline HBV DNA concentrations, HBV genotype A (B), and high baseline ALT levels are significantly associated with treatment response. In patients treated with nucleos(t)ide analogues, low baseline HBV DNA but not viral genotype is positively associated with virologic response. During treatment the best predictor of response is HBV DNA kinetics. Early viral suppression is associated with favourable virologic response and reduced risk for subsequent resistance mutations. For the current standard treatment with pegylated interferon alfa and ribavirin in patients with chronic hepatitis C, infection with HCV genotypes 2 and 3, baseline viral load below 400,000–800,000 IU/ml, Asian and Caucasian ethnicity, younger age, low GGT levels, absence of advanced fibrosis/cirrhosis, and absence of steatosis in the liver have been identified as independent pretreatment predictors of a sustained virologic response. After initiation of treatment, initial viral decline with undetectable HCV-RNA at week 4 of therapy (RVR) is the best predictor of sustained virologic response independent of HCV genotype.
Each lifecycle of the Hepatitis C virus (HCV) produces structural and non-structural (NS) proteins in equimolar. Structural proteins were either assembled or degraded by host proteolysis systems, while NS proteins remain inside the host cells and don’t accumulate. Therefore, they must be degraded. Here, NS3 and NS5A half-lives were quantified in the presence of autolysosome and proteasome different modulators. Inhibitors of both systems increased the half-life, while inducers decreased the half-life. Furthermore, polyubiquitination of NS3 and NS5A was observed. Additionally, their intracellular co-localization with autolysosome (LAMP2) and proteasome (PSMB5) was observed, and inhibitors of both systems increased the degree of co-localization. A better understanding of NS protein degradation might help to improve medical interventions during HCV infections in the future.

In this study, we describe the synthesis of 1,4-disustituted-1,2,3-triazolo-quinazoline ribonucleosides or acyclonucleosides by means of 1,3-dipolar cycloaddition between various O or N-alkylated propargyl-quinazoline and 1'-azido-2',3',5'-tri-O-benzoylribose or activated alkylating agents under microwave conditions. None of the compounds selected showed significant anti-HCV activity in vitro.
Direct acting antivirals (DAAs) revolutionized the therapy of chronic hepatitis C infection. However, unexpected high recurrence rates of hepatocellular carcinoma (HCC) after DAA treatment became an issue in patients with advanced cirrhosis and fibrosis. In this study, we aimed to investigate an impact of DAA treatment on the molecular changes related to HCC development and progression in hepatoma cell lines and primary human hepatocytes. We found that treatment with sofosbuvir (SOF), a backbone of DAA therapy, caused an increase in EGFR expression and phosphorylation. As a result, enhanced translocation of EGFR into the nucleus and transactivation of factors associated with cell cycle progression, B-MYB and Cyclin D1, was detected. Serine/threonine kinase profiling identified additional pathways, especially the MAPK pathway, also activated during SOF treatment. Importantly, the blocking of EGFR kinase activity by erlotinib during SOF treatment prevented all downstream events. Altogether, our findings suggest that SOF may have an impact on pathological processes in the liver via the induction of EGFR signaling. Notably, zidovudine, another nucleoside analogue, exerted a similar cell phenotype, suggesting that the observed effects may be induced by additional members of this drug class.
An accurate quantification of low viremic HCV RNA plasma samples has gained importance since the approval of direct acting antivirals and since only one single measurement predicts the necessity of a prolonged or shortened therapy. As reported previously, HCV quantification assays such as Abbott RealTime HCV and Roche COBAS AmpliPrep/COBAS TaqMan HCV version 2 (CTM v2) may vary in sensitivity and precision particularly in low-level viremia. Importantly, substantial variations were previously demonstrated between some of these assays compared to the Roche High Pure System/COBAS TaqMan assay (HPS) reference assay, which was used to establish the clinical decision points in clinical studies. In this study, the reproducibility of assay performances across several laboratories was assessed by analysing quantification results generated by six independent laboratories (3× RealTime, 3× CTM v2) in comparison with one HPS reference laboratory. The 4th WHO Standard was diluted to 100, 25 and 10 IU/ml, and aliquots were tested in triplicates in 5 independent runs by each assay in the different laboratories to assess assay precision and detection rates. In a second approach, 2 clinical samples (GT 1a & GT 1b) were diluted to 100 and 25 IU/ml and tested as described above. While the result range for WHO 100 IU/ml replicates across all laboratories was similar in this analysis, the CVs of each laboratory ranged from 19.3 to 25.6 % for RealTime laboratories and were lower than CVs of CTM v2 laboratories with a range of 26.1–47.3 %, respectively, and also in comparison with the CV of the HPS reference laboratory (34.9 %). At WHO standard dilution of 25 IU/ml, 24 replicates were quantified by RealTime compared to 8 replicates with CTM v2. Results of clinical samples again revealed a higher variation of CTM v2 results as compared to RealTime values. (CVs at 100 IU/ml: RealTime: 13.1–21.0 % and CTM v2: 15.0–32.3 %; CVs at 25 IU/ml: RealTime 17.6–34.9 % and CTM v2 28.2–54.9 %). These findings confirm the superior precision of RealTime versus CTM v2 at low-level viremia even across different laboratories including the new clinical decision point at 25 IU/ml. A highly precise monitoring of HCV viral load during therapy will remain crucial for patient management with regard to futility rules, therapy efficacy and SVR.
The human immunodeficiency virus (HIV) had spread unrecognized in the human population as sexually transmitted disease and was finally identified by its disease AIDS in 1981. Even after the isolation of the causative agent in 1983, the burden and death rate of AIDS accelerated worldwide especially in young people despite the confection of new drugs capable to inhibit virus replication since 1997. However, at least in industrialised countries, this trend could be reversed by the introduction of combination therapy strategies. The design of new drugs is on going; besides the inhibition of the three enzymes of HIV for replication and maturation (reverse transcriptase, integrase and protease), further drugs inhibits fusion of viral and cellular membranes and virus maturation. On the other hand, viral diagnostics had been considerably improved since the emergence of HIV. There was a need to identify infected people correctly, to follow up the course of immune reconstitution of patients by measuring viral load and CD4 cells, and to analyse drug escape mutations leading to drug resistance. Both the development of drugs and the refined diagnostics have been transferred to the treatment of patients infected with hepatitis B virus (HBV) and hepatitis C virus (HCV). This progress is not completed; there are beneficial aspects in the response of the scientific community to the HIV burden for the management of other viral diseases. These aspects are described in this contribution. Further aspects as handling a stigmatising disease, education of self-responsiveness within sexual relationships, and ways for confection of a protective vaccine are not covered.
Mit 71 Millionen chronisch erkrankten Patienten im Jahr 2015 stellt die chronische Hepatitis C-Virusinfektion eine wichtige Ursache für Zirrhose, Leberdekompensation und Leberkrebs dar.
Eine grundlegende Eigenschaft des Hepatitis C-Virus (HCV) ist die Biogenese modifizierter intrazellulärer Membranen. Das dabei aus dem endoplasmatischen Reticulum (ER) gebildete, sogenannte membranöse Netz (MW, membranous web) dient im Rahmen des HCV-Lebenszyklus als Gerüst für die Assemblierung eines Multi-Protein-Replikase-Komplexes. Das MW wird durch virale nicht-strukturelle Proteine wie NS5A induziert.
Das Multidomänen-Metalloprotein NS5A ist über seine verschiedenen Domänen sowohl bei der Replikation am MW als auch bei der viralen Assemblierung und Freisetzung in der Nähe von Lipidtropfen (LD, lipid droplet) maßgeblich beteiligt. Seine N-terminale amphipathische Helix (AH) spielt dabei über die vermittelte Assoziation von NS5A mit Membranen eine wichtige Rolle. Damit verbundene spezifische Lipidinteraktionen von NS5A unterliegen molekularen Umstrukturierungen, die benötigt werden, um NS5A für seine
verschiedenen Aufgaben im viralen Lebenszyklus anzupassen. Es liegen zwar Röntgen-strukturmodelle von Domäne 1-Dimeren und NMR-Strukturen zur AH vor, allerdings keine experimentellen Strukturen des NS5A-Proteins vollständiger Länge (NS5A fl) in seinem natürlichen Lipidmilieu. Trotz der essentiellen Bedeutung von NS5A für den HCV-Lebens-zyklus und langjähriger Forschung ist bisher nur wenig zur molekularen Funktionsweise von NS5A bekannt.
Dennoch konnten durch Screening hochpotente NS5A-Inhibitoren entdeckt und weiterentwickelt werden. NS5A-Inhibitoren tragen als direkt wirkende antivirale Arzneimittel(DAA, direct-acting antiviral) entscheidend zum Therapieerfolg bei der Behandlung der Hepatitis C bei. Trotz ihrer Bedeutung in der Therapie und intensiver Forschung ist der Wirk-mechanismus von NS5A-Inhibitoren bisher ungeklärt. Eine durch NS5A-Inhibitoren
induzierte intrazelluläre Umverteilung von NS5A und das ausschließliche Auftreten von Resistenz-assoziierten Mutationen (RAM) nahe der NS5A-Lipid-Interaktionsbereiche weisen jedoch auf einen Effekt der Inhibitoren auf die Lipid-NS5A-Interaktion hin.
Als grundlegende Hypothese dieser Arbeit wurde somit vermutet, dass abhängig vom NS5A umgebenden Lipidmilieu (MW oder LD) spezifische Lipid-Protein-Interaktionen Einfluss auf die Struktur und Funktion von NS5A nehmen und NS5A-Inhibitoren über eine Inhibition dieser Interaktionen wirken. Polyphosphoinositide (PPI) könnten dabei als Lipidinteraktionspartner eine besondere Rolle spielen, da sie bedeutend für die Membran-Kennzeichnung verschiedener Zellkompartimente sind und auch die Funktion von Membranproteinen regulieren können. Eine Interaktion von NS5A mit PtdIns(4,5)P2 wurde bereits publiziert.
Um basierend auf der postulierten Hypothese die mechanistischen Details im Wechselspiel von NS5A und intrazellulären Membranen sowie den dabei möglichen Effekt von
NS5A-Inhibitoren zu untersuchen, musste zunächst NS5A in ausreichender Menge, Reinheit und Qualität rekombinant hergestellt werden. Hierfür wurde ein entsprechendes Protokoll zur Proteinexpression durch Baculovirus-vermittelte Expression in Sf9-Insektenzellen und Strep-Tactin-Reinigung für das full length Protein und trunkierte Varianten etabliert.
In Kooperation mit einem Partner konnte unter Verwendung von giant unilamellar vesicles (GUVs) und konfokaler Mikroskopie gezeigt werden, dass unser full length Protein die Struktur von Membranen verändert (Membran-Remodellierung).
Die Stabilität des gereinigten Proteins und damit Effekte auf die Proteinfaltung wurden mittels Thermal shift assay (TSA) untersucht und dabei auch Effekte des NS5A-Inhibitors
Daclatasvir (DCV) und des Metall-Chelators EDTA überprüft. Die Bindung des Inhibitors hatte einen stabilisierenden Effekt auf die Proteinstruktur zur Folge.
Potentielle Interaktionsmuster mit Membranlipiden wurden mit Hilfe eines Protein lipid overlay assays (PLOA) detektiert. Zusätzlich zum in der Literatur bereits beschriebenen
Interaktionspartner PtdIns(4,5)P2 konnten weitere Lipid-Bindungspartner für NS5A identifiziert werden. Dabei legen die gewonnenen Daten nahe, dass die Interaktion über die Domäne 1 von NS5A vermittelt wird, wobei die Domänen 2 und 3 die Affinität zu den Lipidbindungspartnern erhöht, aber nicht das Phospholipid-Bindungsmuster verändert.
DCV hatte im PLOA keine qualitativen Auswirkungen auf das Lipid-Bindungsmuster. Die Lipidinteraktionen wurden mittels eines Liposomen-Rekonstitutionsmodells validiert.
In silico konnten basierend auf verfügbaren, experimentellen Strukturdaten und einem dynamischen Modell drei Cluster basischer Aminosäuren in NS5A-D1-AH als mögliche
PPI-Bindungsstellen identifiziert werden. Basierend auf dem Strukturmodell wurde eine Mutationsstrategie zur Charakterisierung der potentiellen PPI-Bindungsstellen entwickelt.

Although direct-acting antiviral medications effectively cure hepatitis C in most patients, sometimes treatment selects for resistant viruses, causing antiviral drugs to be either ineffective or only partially effective. Multidrug resistance is common in patients for whom DAA treatment fails. Older patients and patients with advanced liver diseases are more likely to select drug-resistant viruses. Collective efforts from international communities and governments are needed to develop an optimal approach to managing drug resistance and preventing the transmission of resistant viruses.
Behandlung von Hepatitis-C-Infektionen im Zeitalter direkt wirkender antiviraler Medikamente (DAAs)

Die chronische Hepatitis-C-Infektion kann unbehandelt zu schwerwiegenden und potenziell lebensbedrohlichen leberassoziierten Komplikationen führen. Grundsätzlich stellt damit jede chronische Infektion mit dem Hepatitis-C-Virus (HCV) eine Indikation zur antiviralen Therapie dar. Besonders dringlich ist sie jedoch bei Patient*innen mit fortgeschrittener Lebererkrankung. In diesem Beitrag werden Indikation, Therapieziele und Grundprinzipien der direkt antiviralen Therapie beschrieben. Verschiedene Therapieregime und Möglichkeiten der Überwachung von Therapie und Therapieerfolg werden vorgestellt.
Heutzutage wird die chronische HCV-Infektion interferonfrei mit direkt antiviral wirksamen Medikamenten („direct acting antivirals“ – DAA) behandelt, wobei die Wahl der Medikamente von HCV-Genotyp, Vortherapie und Fibrosestatus abhängt. Patient*innen mit kompensierter Leberzirrhose und solche ohne Leberzirrhose weisen unter Behandlung vergleichbar hohe Viruseradikationsraten auf. Auch bei dekompensierter Leberzirrhose oder dialysepflichtiger Niereninsuffizienz und selbst bei Kindern ab einem Alter von 3 Jahren ist heutzutage eine sichere und hocheffiziente antivirale Behandlung möglich. Medikamenteninteraktionen sind zu beachten, können aber einfach und schnell im Internet überprüft werden. Auch wenn sich die Prognose nach HCV-Eradikation deutlich verbessert, sollten Patient*innen mit fortgeschrittener Leberfibrose bzw. einer Leberzirrhose lebenslang weiterbeobachtet werden, um die Entstehung eines hepatozellulären Karzinoms rechtzeitig zu erkennen (HCC-Surveillance).
