Background: To investigate patients’ perspectives on polypharmacy and the use of a digital decision support system to assist general practitioners (GPs) in performing medication reviews. Methods: Qualitative interviews with patients or informal caregivers recruited from participants in a cluster-randomized controlled clinical trial (cRCT). The interviews were transcribed verbatim and analyzed using thematic analysis. Results: We conducted 13 interviews and identified the following seven themes: the patients successfully integrated medication use in their everyday lives, used medication plans, had both good and bad personal experiences with their drugs, regarded their healthcare providers as the main source of medication-related information, discussed medication changes with their GPs, had trusting relationships with them, and viewed the use of digital decision support tools for medication reviews positively. No unwanted adverse effects were reported. Conclusions: Despite drug-related problems, patients appeared to cope well with their medications. They also trusted their GPs, despite acknowledging polypharmacy to be a complex field for them. The use of a digital support system was appreciated and linked to the hope that reasons for selecting specific medication regimens would become more comprehensible. Further research with a more diverse sampling might add more patient perspectives.
'Skelettfund' im Keller

Bei der Identifizierung einer unbekannten, stark verwesten Leiche oder eines Skelettes ohne Hinweise auf die Identität durch die Auffindesituation spielt die Erstellung des sog. biologischen Profils eine entscheidende Rolle. Vorgestellt wird ein Leichenfund in einem mehr oder weniger frei zugänglichen Kellerabteil eines Mehrfamilienhauses. Der Leichnam war weitgehend skelettiert, das Skelett jedoch durch mumifizierte Weichteilreste noch nahezu vollständig zusammengehalten. Bei den Hinweisen auf die Identität ergaben sich insbesondere in der Altersschätzung scheinbare Widersprüche, die jedoch zufällig eine relativ genau zutreffende Schätzung lieferten. Die Überreste konnten mittels forensischer DNA-Analyse einer seit 4 Jahren vermissten 49-Jährigen zugeordnet werden. Als Todesursache wurde ein Kältetod diskutiert.
Der Fall wies eine außergewöhnliche Auffindesituation auf, die an Bilder von „Skelettfunden“ in Kriminalverfilmungen erinnerte. Derartige Befunde dürften jedoch in der Realität wohl nur sehr selten vorkommen. Darüber hinaus werden die Wichtigkeit und die Probleme der forensisch-osteologischen Untersuchungen bei der Identifizierung eines stark verwesten, unbekannten Leichnams demonstriert.
Background: The rate of caesarean sections increased in the last decades to about 30% of births in western populations. Many caesarean sections are electively planned without an urgent medical reason for mother or child. Especially in women with a foregoing caesarean section, the procedure is planned early. An early caesarean section though may harm the newborn. Our aim is to evaluate the gestational time point after the 37th gestational week (after prematurity = term) of performing an elective caesarean section with the lowest morbidity for mother and child.
Methods: This is an update of a systematic review previously carried out on behalf of the German Federal Ministry of Health. We will perform a systematic literature search in MEDLINE, EMBASE, CENTRAL and CINAHL. Our primary outcome is the rate of admissions to the neonatal intensive care unit in early versus late term neonates. We will include (quasi) randomized controlled trials and cohort studies. The studies should include pregnant women who have an elective caesarean section at term. We will screen titles and abstracts and the identified full texts of studies for eligibility. Risk of bias will be assessed with the Cochrane Risk of Bias Tool for Randomized Trials or with the Risk of Bias Tool for Non-Randomized Studies of Interventions (ROBINS-I). These tasks will be performed independently by two reviewers. Data will be extracted in beforehand piloted extraction tables. A dose-response meta-analysis will be performed.
Discussion: Our aim is to reach a higher validity in the assessment of the time point of elective caesarean sections by performing a meta-analysis to support recommendations for clinical practice. We assume to identify less randomized controlled trials but a large number of cohort studies analyzing the given question. We will discuss similarities and differences in included studies as well as methodological strengths and weaknesses.

Die in vivo MR-Protonenspektroskopie (1H-MRS) ist ein nichtinvasives Verfahren, das die Untersuchung biochemischer Substanzen beim Lebenden ermöglicht. In einer prospektiven Studie wurden Patienten, die an einer Demenz vom Alzheimer-Typ (DAT) litten, und altersentsprechende Probanden mit der 1H-MRS untersucht. Im Hirngewebe wurden Konzentrationen von Molekülen bestimmt, die Aussagen über die zelluläre Zusammensetzung ermöglichen. Das Hauptinteresse galt der Untersuchung von Glutamat und Glutamin, da ein Überangebot des Neurotransmitters Glutamat im synaptischen Spalt möglicherweise für die Neurodegeneration beim M. Alzheimer mitverantwortlich ist. Bei 29 Patienten mit einer DAT sowie 19 Probanden wurden zwei Volumina in Kortex und Marklager des Parietallappens bei 1,5 Tesla Magnetfeldstärke mit einer PRESS-Sequenz (TE = 22 ms, TR = 3 s) untersucht. Die Patienten waren kognitiv leicht bis mittelschwer beeinträchtigt (Mini Mental State Examination, MMSE, 11-27 Punkte). Die quantitative Auswertung der Spektren erfolgte nach der "phantom replacement method" mit dem Softwareprogramm LCModel. Als Kriterium für die Zuverlässigkeit der Konzentrationsangaben diente die vom Programm als %SD-Wert angegebene Auswertegenauigkeit der Einzelmessungen. Neben dem Glutamat- und Glutamingehalt wurden der Liquorgehalt im kortikalen Messvolumen und die Konzentrationen der Moleküle N-Acetyl-Aspartat und N-Acetyl-Aspartyl- Glutamat (tNAA), myo-Inositol (mI), Kreatin und Kreatinphosphat (tCr) sowie der cholinhaltigen Substanzen (tCh) bestimmt. Bei den Patienten konnten 25 aussagekräftige Spektren aus der Rindenregion und 19 aus dem Marklager des Parietallappens gemessen werden, bei den Probanden 17 Kortex- und 13 Marklager-Spektren. Glutamat und Glutamin können mit der in vivo-Spektroskopie wegen ihrer sich überlappenden Resonanzen nicht separat bestimmt werden und wurden deshalb als „Glx“ zusammengefasst. Der Glx-Gehalt betrug in der Rindenregion bei den Patienten im Durchschnitt 15,48 ± 5,15 mmol/l, bei den Probanden 13,98 ± 3,14 mmol/l. Die um 11% höhere Konzentration bei den Patienten war nicht signifikant. Die Glx-Konzentrationen im Marklager betrugen 8,16 ± 3,79 mmol/l bei den Patienten und 8,14 ± 3,71 mmol/l bei den Probanden. Nach dem Kriterium der %SD war die Zuverlässigkeit der Konzentrationsbestimmung von Glutamat und Glutamin eingeschränkt, was auf die protonenspektroskopischen Eigenschaften von Glutamat und Glutamin zurückzuführen ist. Sie erzeugen wegen ihrer gekoppelten Resonanzen breite und mehrgipflige Protonensignale, die wegen der resultierenden niedrigen Signalamplitude für LCModel von Makromolekülresonanzen oder von Artefakten, die durch ungenügende Wasserunterdrückung entstehen, schwierig abzugrenzen sind. Die %SD-Werte der Einzelmessungen von tNAA, mI, tCr und tCh waren gering, die Messergebnisse können daher als zuverlässig angesehen werden. Die tNAA-Konzentrationen in beiden Gruppen unterschieden sich weder in der grauen noch in der weißen Substanz signifikant voneinander. Im Hirnrindenvolumen zeigte sich bei den Patienten jedoch eine positive Korrelation von tNAA und MMSE-Ergebnis, also eine Reduktion von tNAA mit zunehmenden kognitiven Einbußen. Dies spricht für eine Abnahme des neuronalen Volumenanteils mit fortschreitender Erkrankung. Der durchschnittliche myo-Inositolgehalt im Marklagervolumen lag in der Patientengruppe um 20% höher als bei den Probanden. Der Unterschied erreichte jedoch kein Signifikanzniveau (p = 0,09). Im Rindenvolumen war die mittlere myo-Inositolkonzentration bei den Patienten um 7% höher als in der Kontrollgruppe, auch hier war der Unterschied nicht signifikant. Der nichtsignifikante myo-Inositolanstieg lässt sich als mäßige entzündliche oder reaktive Gliaproliferation interpretieren. Die tCr-Konzentrationen in beiden Gruppen unterschieden sich nicht, jedoch bestand bei den DAT-Patienten eine positive Korrelation des tCr-Gehalts mit der MMSE-Punktzahl in der grauen Substanz des Parietallappens, die Konzentration nahm also mit zunehmendem Schweregrad der Demenz ab. Bei der durchschnittlichen tCh-Konzentration bestanden keine Gruppenunterschiede in den untersuchten Regionen. Die Patienten wiesen eine signifikante Zunahme des mittleren Liquoranteils um 16% im kortikalen Volumen auf. Dies ist als kortikale Hirngewebeatrophie in frühen und mittleren Stadien der DAT zu werten. Während die in vivo-Konzentrationen von tNAA, mI, tCr und tCh zuverlässig bestimmt werden konnten, war die Messung von Glutamat und Glutamin bei 1,5 T mit technischen Schwierigkeiten verbunden. Wegen der hohen Standardabweichungen konnte aus dieser Untersuchung keine gesicherte Aussage zu einer möglichen Konzentrationsänderung dieser Aminosäuren bei der DAT abgeleitet werden. Es ist zu erwarten, dass hier durch MR-Geräte mit 3 T Feldstärke sowie durch den Einsatz der parallelen Bildgebung ein erheblich höheres Signal/Rausch-Verhältnis und damit genauere Ergebnisse erzielt werden können.
Ziel der Nachuntersuchung war es, 10 Jahres Ergebnisse nach arthroskopischer vorderer Kreuzbandplastik mittels Patellarsehne zu erhalten. Dafür wurden 74 Patienten, 55 Männer und 19 Frauen mit einem durchschnittlichen Alter von 38 Jahren im Mittel 113 Monaten nach arthroskopischer vorderer Kreuzbandplastik mit Interferenzschraubenfixation nachuntersucht. Evaluiert wurden die IKDC Evaluation, die Tegner - Aktivitätsskala und der Lysholm - Score. Die Bandstabilität wurde instrumentell mit dem Rolimeter getestet. Die statistische Auswertung erfolgte mittels c² Test. Die Tegner - Aktivitätsskala ergab ein durchschnittliches Aktivitätsniveau von 5,2. Vor der Verletzung lag der Wert bei 6,4. Der Lysholm - Score erreichte 93,6 von 100 Punkten. Hinsichtlich der Funktion wurden 74% und hinsichtlich der Stabilität 72% der Patienten in der IKDC Evaluation als normal bzw. als fast normal bewertet. Der mit dem Rolimeter bestimmte Lachman Test war in 88% normal und in 12% fast normal. Der pivot-shift Test war in 95% der Fälle negativ. 30% der Patienten zeigten in den Röntgenaufnahmen zum Nachuntersuchungszeitpunkt deutlich degenerative Veränderungen. Es ergibt sich eine signifikante Korrelation zwischen der Zeitdauer vom Unfall bis zur operativen Versorgung und dem Arthrosegrad. Die Beschwerden an der Transplantatentnahmestelle korrelieren signifikant mit den allgemeinen Symptomen. Das Ausmaß der Arthrose korreliert signifikant mit der Mensikusschädigung. Zur Vermeidung der Folgearthrose sollte daher bei gegebener Indikation eine frühzeitige Kreuzbandversorgung und im Falle der Begleitmeniskusverletzung dessen Erhalt mittels Refixation angestrebt werden.
100 Jahre Dieter Janz

The 20 April 2020 marks the centenary of Dieter Janz’s birth. This issue of Zeitschrift für Epileptologie is published in his honor with the aim of tracing the work of Dieter Janz over the last five decades and summarizing new findings on the Janz syndrome (Juvenile Myoclonic Epilepsy), which is named after him.
Introduction: Epoxyeicosatrienoic acids (EETs) are able to enhance angiogenesis and regulate inflammation that is especially important in wound healing under ischemic conditions. Thus, we evaluated the effect of local EET application on ischemic wounds in mice.
Methods: Ischemia was induced by cautherization of two of the three supplying vessels to the mouse ear. Wounding was performed on the ear three days later. Wounds were treated either with 11,12 or 14,15 EET and compared to untreated control and normal wounds. Epithelialization was measured every second day. VEGF, TNF-α, TGF-β, matrix metalloproteinases (MMP), tissue inhibitors of metalloproteinases (TIMP), Ki67, and SDF-1α were evaluated immunohistochemically in wounds on day 3, 6, and 9.
Results: Ischemia delayed wound closure (12.8 days ± 1.9 standard deviation (SD) for ischemia and 8.0 days ± 0.94 SD for control). 11,12 and14,15 EET application ameliorated deteriorated wound healing on ischemic ears (7.6 ± 1.3 SD for 11,12 EET and 9.2 ± 1.4 SD for 14,15 EET). Ischemia did not change VEGF, TNF-α, TGF-β, SDF-1α, TIMP, MMP7 or MMP9 level significantly compared to control. Local application of 11,12 as well as 14,15 EET induced a significant elevation of VEGF, TGF-β, and SDF-1α expression as well as proliferation during the whole phase of wound healing compared to control and ischemia alone.
Conclusion: In summary, EET improve impaired wound healing caused by ischemia as they enhance neovascularization and alter inflammatory response in wounds. Thus elevating lipid mediator level as 11,12 and 14,15 EET in wounds might be a successful strategy for amelioration of deranged wound healing under ischemia.
Epoxyeicosatrienoic acids (EET) facilitate regeneration in different tissues, and their benefit in dermal wound healing has been proven under normal conditions. In this study, we investigated the effect of 11,12 EET on dermal wound healing in diabetes. We induced diabetes by i.p. injection of streptozotocin 2 weeks prior to wound creation on the dorsal side of the mouse ear. 11,12 EET was applied every second day on the wound, whereas the control groups received only solvent. Epithelialization was monitored every second day intravitally up to wound closure. Wounds were stained for VEGF, CD31, TGF-β, TNF-α, SDF-1α, NF-κB, and Ki-67, and fibroblasts were counted after hematoxylin-eosin stain on days 3, 6, 9, and 16 after wounding. After induction of diabetes, wounds closed on day 13.00 ± 2.20 standard deviation (SD). Local 11,12 ETT application improved wound closure significantly to day 8.40 ± 1.39 SD. EET treatment enhanced VEGF and CD31 expression in wounds on day 3. It also seemed to raise TNF-α level on all days investigated as well as TGF-β level on days 3 and 6. A decrease in NF-κB could be observed on days 9 and 16 after EET application. The latter findings were not significant. SDF-1α expression was not influenced by EET application, and Ki-67 was significantly less in the EET group on day 9 after EET application. The number of fibroblasts was significantly increased on day 9 after the 11,12 EET application. 11,12 EET improve deteriorated wound healing in diabetes by enhancing neoangiogenesis, especially in the early phase of wound healing. Furthermore, they contribute to the dissolution of the initial inflammatory reaction, allowing the crucial transition from the inflammatory to proliferative phase in wound healing.
In the systemic circulation, 11,12-epoxyeicosatrienoic acid (11,12-EET) elicits nitric oxide (NO)- and prostacyclin-independent vascular relaxation, partially through the activation of large conductance Ca2+-activated potassium (BK) channels. However, in the lung 11,12-EET contributes to hypoxia-induced pulmonary vasoconstriction. Since pulmonary artery smooth muscle cells also express BK channels, we assessed the consequences of BKβ1 subunit deletion on pulmonary responsiveness to 11,12-EET as well as to acute hypoxia. In buffer-perfused mouse lungs, hypoxia increased pulmonary artery pressure and this was significantly enhanced in the presence of NO synthase (NOS) and cyclooxygenase (COX) inhibitors. Under these conditions the elevation of tissue EET levels using an inhibitor of the soluble epoxide hydrolase (sEH-I), further increased the hypoxic contraction. Direct administration of 11,12-EET also increased pulmonary artery pressure, and both the sEH-I and 11,12-EET effects were prevented by iberiotoxin and absent in BKβ1−/− mice. In pulmonary artery smooth muscle cells treated with NOS and COX inhibitors and loaded with the potentiometric dye, di-8-ANEPPS, 11,12-EET induced depolarization while the BK channel opener NS1619 elicited hyperpolarization indicating there was no effect of the EET on classical plasma membrane BK channels. In pulmonary artery smooth muscle cells a subpopulation of BK channels is localized in mitochondria. In these cells, 11,12-EET elicited an iberiotoxin-sensitive loss of mitochondrial membrane potential (JC-1 fluorescence) leading to plasma membrane depolarization, an effect not observed in BKβ1−/− cells. Mechanistically, stimulation with 11,12-EET time-dependently induced the association of the BK α and β1 subunits. Our data indicate that in the absence of NO and prostacyclin 11,12-EET contributes to pulmonary vasoconstriction by stimulating the association of the α and β1 subunits of mitochondrial BK channels. The 11,12-EET-induced activation of BK channels results in loss of the mitochondrial membrane potential and depolarization of the pulmonary artery smooth muscle cells.
