Proton-translocating NADH:ubiquinone oxidoreductase (complex I) is the largest and least understood enzyme of the respiratory chain. Complex I from bovine mitochondria consists of more than forty different polypeptides. Subunit PSST has been suggested to carry iron-sulfur center N-2 and has more recently been shown to be involved in inhibitor binding. Due to its pH-dependent midpoint potential, N-2 has been proposed to play a central role both in ubiquinone reduction and proton pumping. To obtain more insight into the functional role of PSST, we have analyzed site-directed mutants of conserved acidic residues in the PSST homologous subunit of the obligate aerobic yeast Yarrowia lipolytica. Mutations D136N and E140Q provided functional evidence that conserved acidic residues in PSST play a central role in the proton translocating mechanism of complex I and also in the interaction with the substrate ubiquinone. When Glu89, the residue that has been suggested to be the fourth ligand of iron-sulfur center N-2 was changed to glutamine, alanine, or cysteine, the EPR spectrum revealed an unchanged amount of this redox center but was shifted and broadened in the gzregion. This indicates that Glu89 is not a ligand of N-2. The results are discussedin the light of structural similarities to the homologous [NiFe] hydrogenases.
Ziel unserer retrospektiven Studie war es, die refraktiven und kornealen Veränderungen nach DMEK bei pseudophaken Patienten, die sich auf Grund einer endothelialen Hornhauterkrankung behandeln ließen, zu untersuchen. Durch unsere einheitlich pseudophake Patientenkohorte wollten wir untersuchen, ob sich die refraktiven Veränderungen nach DMEK von den bereits bekannten Änderungen bei einer simultan durchgeführten Katarakt- und DMEK-Operation sogenannte „Triple“-DMEK unterscheiden. Primärer Endpunkt der Studie war die Veränderung der Refraktion unter besonderer Berücksichtigung des sphärischen Äquivalents (SEQ) des jeweiligen pseudophaken Auges nach DMEK. Sekundäre Endpunkte umfassten die Entwicklung des Visus, der CCT, der ECD und verschiedener kornealer Parameter, die mittels Scheimpflug- Tomographie ermittelt wurden.
In der vorliegenden Arbeit erfolgte hierzu die retrospektive Analyse von Daten, die in den Patientenakten dokumentiert und digital gespeichert waren (Pentacam® HR). Es wurden 109 Augen von 95 Patienten, die sich im Zeitraum von Februar 2015 bis Dezember 2018 mittels DMEK in unserem Zentrum behandeln ließen, in die Studie eingeschlossen. Davon stammten 66 Augen (61%) von weiblichen und 43 Augen (39%) von männlichen Patienten. Es handelte sich bei 61 Augen (56%) um ein linkes und bei 48 Augen (44 %) um ein rechtes Auge. Die Patienten waren 20 bis 91 Jahre alt. Das mittlere Alter zum Zeitpunkt der DMEK-Operation betrug 71,9 Jahre (SD ±10,23). Der Altersmittelwert der Männer lag bei 70,4 Jahren (SD 11,23, Spannweite: 20-84) und der der Frauen bei 72,9 Jahren (SD 9,49, Spannweite: 47-91). Der mittlere Nachbeobachtungszeitraum betrug 10,47 Monate (SD 6,78, Spannweite: 1-28 Monate).
Für das SEQ konnte bei Betrachtung aller ausgewerteten Daten eine leichte Tendenz in Richtung eines „hyperopen shifts“ mit einer mittleren Veränderung des SEQ von + 0,1 D gezeigt werden, die jedoch nicht statistisch signifikant war. Zwischenzeitlich kam es im Nachbeobachtungszeitraum zu einer klaren Tendenz hinsichtlich einer Myopisierung bei Betrachtung aller ausgewerteten Daten. Der „hyperope shift“ konnte erst am Ende, möglicherweise infolge einer deutlich reduzierten Patientenzahl, beobachtet werden. In der Subgruppe „vollständige Kontrollen“ für das SEQ zeigte sich eine Tendenz hinsichtlich einer leichten Abnahme des SEQ, die jedoch ebenfalls nicht statistisch signifikant war und aufgrund der geringen Gruppengröße (n=32) kritisch betrachtet werden sollte. In unserer Kohorte konnte somit keine eindeutige Aussage über eine postoperative Änderung des SEQ in Richtung eines „hyperopen shifts“ oder „myopen shifts“ gemacht werden. Die Refraktion verhielt sich in unserem Patientenkollektiv nach DMEK insgesamt weitestgehend stabil. Insofern sind refraktive "Überraschungen", wie sie weiterhin häufig nach "Triple"-DMEK zu beobachten sind, bei zuvor pseudophakisierten Patienten in einem weitaus geringeren Maße zu erwarten. Unter den mittels Scheimpflug-Technologie untersuchten kornealen Parametern wies lediglich der posteriore Astigmatismus signifikante Veränderungen im Sinne einer Reduktion der kornealen Krümmung auf. Indirekt heben unsere Ergebnisse damit die Bedeutung des posterioren Hornhautprofils auf die postoperative Refraktionsentwicklung und somit auch auf die IOL-Kalkulation bei "Triple"-DMEK-Prozeduren hervor. Außerdem scheint der postoperative Anstieg der Sehschärfe mit den gleichfalls signifikanten Änderungen der kornealen Densitometrie in der 2-6 mm Zone, des Hornhautvolumens und der zentralen Hornhautdicke umgekehrt korreliert zu sein.
Darüber hinaus lässt sich festhalten, dass auch unsere Untersuchung in einem gewissen Maße die Überlegenheit der DMEK gegenüber etablierten Techniken, wie beispielsweise der PK verdeutlicht. Sowohl anhand der refraktiven Stabilität als auch der Visusergebnisse konnten wir belegen, dass das DMEK-Verfahren nach der anfänglichen Lernkurve die wohl besten funktionellen Ergebnisse in der Behandlung von Patienten mit endothelialen Hornhauterkrankungen liefert. Besonders bei bereits pseudophaken Patienten weist die DMEK durch die zu erwartende hohe postoperative refraktive Stabilität viele Vorteile auf
und erscheint insbesondere hinsichtlich der Vorhersagbarkeit des postoperativen refraktiven Ergebnisses der „Triple“- DMEK überlegen.
In der vorliegenden Studie wurden Patienten mit struktureller Epilepsie bedingt durch eine fokale kortikale Dysplasie (FCD) mittels moderner Magnetresonanztomographie (MRT)-Verfahren untersucht.
Bei FCDs handelt es sich um Fehlbildungen der Großhirnrinde, die mit einer hohen epileptogenen Aktivität vergesellschaftet sind. Einige dieser Patienten unterziehen sich einer epilepsiechirurgischen Resektion, sind jedoch hiernach hinsichtlich ihrer Anfallsfrequenz dennoch nicht ausreichend kontrollierbar, weshalb Grund zur Annahme besteht, dass es neben der fokalen kortikalen Dysplasie andere Faktoren geben könnte, die epileptische Anfälle verursachen.
Basierend auf dieser Überlegung wurde mittels T2-Relaxometrie untersucht, ob bei Patienten mit FCDs mikrostrukturelle Veränderungen in Teilen des Kortex vorhanden sind, die mittels konventioneller MRT-Verfahren normal bzw. gesund erscheinen. Es wird angenommen, dass bei diesen Patienten auch außerhalb der FCD mikrostrukturelle Veränderungen, beispielsweise bedingt durch Schädigung im Rahmen von Anfällen oder durch Therapieeffekte, vorzufinden sind.
Für die Studie wurden 16 Patienten mit einer neuroradiologisch gesicherten FCD und 16 hinsichtlich des Alters und des Geschlechts gematchte gesunde Probanden rekrutiert.
Die Daten wurden an einem 3 Tesla (T) MRT-Scanner erhoben. Um die T2-Relaxationszeit zu messen, wurden Spin-Echo Datensätze mit verschiedenen Echozeiten (TE) aufgezeichnet. Zur Erfassung der Ausdehnung der FCD wurden konventionelle fluid-attenuated inversion recovery (FLAIR)-Datensätze akquiriert. Zur Segmentierung des Gewebes wurden synthetische T1-gewichtete magnetization-prepared rapid acquisition of gradient echos (MP-RAGE)-Datensätze aus quantitativen T1-Karten berechnet. Der Kortex und dessen Grenzflächen wurden mittels FreeSurfer anhand der MP-RAGE-Datensätze identifiziert und die kortikale Dicke wurde gemessen. Die FCD-Areale wurden in den FLAIR-Datensätzen manuell markiert und aus den T2-Karten exkludiert, um die FCD-assoziierten Veränderungen nicht in die Analyse einzubeziehen.
Anschließend wurden kortikale T2-Werte ausgelesen und in Oberflächendatensätzen gespeichert, um dann durchschnittliche kortikale T2-Werte für jeden Probanden zu ermitteln und mittels ungepaartem t-Test zwischen den Gruppen zu vergleichen. Zudem wurde der Pearson-Korrelationskoeffizient zwischen den kortikalen T2 Werten und klinischen Parametern berechnet. Außerdem wurde eine oberflächenbasierte Gruppenanalyse kortikaler T2-Werte und der kortikalen Dicke durchgeführt. Hierbei wurden Permutationssimulationen durchgeführt, um kortikale Cluster zu erkennen, die fokale Gruppenunterschiede anzeigen, und um für Mehrfachvergleiche zu korrigieren.
Die Analyse ergab, dass die durchschnittlichen kortikalen T2-Werte außerhalb der FCD in der Patientenkohorte im Vergleich zu den gesunden Probanden signifikant erhöht waren. Diese T2-Veränderungen zeigten weder eine signifikante Korrelation mit der Anzahl der Anfälle der letzten drei Monate, noch mit der Anzahl der jemals eingenommenen antiepileptischen Medikamente. Insbesondere wurden T2-Erhöhungen in den frontalen, parietalen und manchen temporalen Regionen festgestellt. Die oberflächenbasierte Analyse der Kortexdicke zeigte keine signifikanten Gruppenunterschiede.
Mittels T2-Relaxometrie und oberflächenbasierten Analyse-Techniken wurden demnach T2-Veränderungen des mittels konventioneller MRT-Bildgebung unauffällig erscheinenden zerebralen Kortex bei Patienten mit FCD und Epilepsie festgestellt.
Die Ergebnisse deuten auf das Vorhandensein von mikrostrukturellen Veränderungen hin, die sich mit konventionellen MRT-Verfahren nicht erfassen lassen. Potentielle Ursachen dieser Veränderungen sind neben Effekten der antikonvulsiven Medikation möglicherweise auch gliotischer Gewebeumbau bedingt durch stattgehabte epileptische Anfälle. Die Studie legt nahe, dass strukturelle Epilepsien mehr als ein Symptom bedingt durch eine fokale Läsion sind und stattdessen das Gehirn als Ganzes betreffen.
Cortical changes in epilepsy patients with focal cortical dysplasia: new insights with T2 mapping

Background: In epilepsy patients with focal cortical dysplasia (FCD) as the epileptogenic focus, global cortical signal changes are generally not visible on conventional MRI. However, epileptic seizures or antiepileptic medication might affect normal-appearing cerebral cortex and lead to subtle damage. Purpose: To investigate cortical properties outside FCD regions with T2-relaxometry. Study Type: Prospective study. Subjects: Sixteen patients with epilepsy and FCD and 16 age-/sex-matched healthy controls. Field Strength/Sequence: 3T, fast spin-echo T2-mapping, fluid-attenuated inversion recovery (FLAIR), and synthetic T1-weighted magnetization-prepared rapid acquisition of gradient-echoes (MP-RAGE) datasets derived from T1-maps. Assessment: Reconstruction of the white matter and cortical surfaces based on MP-RAGE structural images was performed to extract cortical T2 values, excluding lesion areas. Three independent raters confirmed that morphological cortical/juxtacortical changes in the conventional FLAIR datasets outside the FCD areas were definitely absent for all patients. Averaged global cortical T2 values were compared between groups. Furthermore, group comparisons of regional cortical T2 values were performed using a surface-based approach. Tests for correlations with clinical parameters were carried out. Statistical Tests: General linear model analysis, permutation simulations, paired and unpaired t-tests, and Pearson correlations. Results: Cortical T2 values were increased outside FCD regions in patients (83.4 ± 2.1 msec, control group 81.4 ± 2.1 msec, P = 0.01). T2 increases were widespread, affecting mainly frontal, but also parietal and temporal regions of both hemispheres. Significant correlations were not observed (P ≥ 0.55) between cortical T2 values in the patient group and the number of seizures in the last 3 months or the number of anticonvulsive drugs in the medical history. Data Conclusion: Widespread increases in cortical T2 in FCD-associated epilepsy patients were found, suggesting that structural epilepsy in patients with FCD is not only a symptom of a focal cerebral lesion, but also leads to global cortical damage not visible on conventional MRI. Objectives: Inadequate oral hygiene still leads to many serious diseases all over the world. Therefore, this study aimed to analyze scientific research in the field of oral health in order to be able to comprehend their relevant subject areas, research connections, or developments. Methods: This study aimed to assess the global publication output on oral hygiene to create a world map that provides background information on key players, trends, and incentives of research. For this purpose, established bibliometric parameters were combined with state-of-the-art visualization techniques. Results: This study shows the actual key players of research on oral hygiene in high-income economies with only marginal participation from lower economies. This still corresponds to the current burden situations, but they are more and more shifting to the disadvantage of the low-income countries. There is a clear North–South and West–East gradient, with the USA and the Western European nations being the most publishing nations on oral hygiene. As an emerging country, Brazil plays a role in the research. Conclusions: The scientific power players were concentrated in high-income countries. However, the changing epidemiological situation requires a different scientific approach to oral hygiene. This requires an expansion of the international network to meet the demands of future global oral health burdens, which are mainly related to oral hygiene.
Glioblastoma is the most common malignant primary brain tumor. To date, clinically relevant biomarkers are restricted to isocitrate dehydrogenase (IDH) gene 1 or 2 mutations and O6-methylguanine DNA methyltransferase (MGMT) promoter methylation. Long non-coding RNAs (lncRNAs) have been shown to contribute to glioblastoma pathogenesis and could potentially serve as novel biomarkers. The clinical significance of HOXA Transcript Antisense RNA, Myeloid-Specific 1 (HOTAIRM1) was determined by analyzing HOTAIRM1 in multiple glioblastoma gene expression data sets for associations with prognosis, as well as, IDH mutation and MGMT promoter methylation status. Finally, the role of HOTAIRM1 in glioblastoma biology and radiotherapy resistance was characterized in vitro and in vivo. We identified HOTAIRM1 as a candidate lncRNA whose up-regulation is significantly associated with shorter survival of glioblastoma patients, independent from IDH mutation and MGMT promoter methylation. Glioblastoma cell line models uniformly showed reduced cell viability, decreased invasive growth and diminished colony formation capacity upon HOTAIRM1 down-regulation. Integrated proteogenomic analyses revealed impaired mitochondrial function and determination of reactive oxygen species (ROS) levels confirmed increased ROS levels upon HOTAIRM1 knock-down. HOTAIRM1 knock-down decreased expression of transglutaminase 2 (TGM2), a candidate protein implicated in mitochondrial function, and knock-down of TGM2 mimicked the phenotype of HOTAIRM1 down-regulation in glioblastoma cells. Moreover, HOTAIRM1 modulates radiosensitivity of glioblastoma cells both in vitro and in vivo. Our data support a role for HOTAIRM1 as a driver of biological aggressiveness, radioresistance and poor outcome in glioblastoma. Targeting HOTAIRM1 may be a promising new therapeutic approach.
Fallzahlaufkommen und Qualitätsindikatoren bei der Versorgung des abdominellen Bauchaortenaneurysmas

Hintergrund: Der MTL30 ist ein Surrogatparameter, der bisher in visceralchirurgischen Registern verwendet wird, um eine Prognose über die Qualität verschiedener komplikationsträchtiger Eingriffe abzugeben. Der Marker wird als positiv gewertet, wenn ein Patient am 30. postoperativen Tag verstorben (mortality) ist, in ein anderes Akutkrankenhaus verlegt (transfer) werden musste oder weiterhin im Krankenhaus verweilen muss (length of stay >30 days).
Zielsetzung: In dieser Arbeit sollte untersucht werden, ob der MTL30 eine Aussage über die Qualität der operativen Versorgung des Bauchaortenaneurysmas (AAA) abgeben kann. Dazu sollten Kliniken mit unterschiedlichem Fallaufkommen hinsichtlich Letalität und MTL30 miteinander verglichen werden. Zusätzlich sollten Literaturempfehlungen zu Mindestmengen des Fallaufkommens überprüft werden.
Methodik: Insgesamt wurden 18.205 elektive Eingriffe aus dem AAA- Register des Deutschen Instituts für Gefäßmedizinische Gesundheitsforschung (DIGG) der Deutschen Gesellschaft für Gefäßchirurgie und Gefäßmedizin (DGG) analysiert. 14.282 Patienten wurden mittels endovaskulärer Aortenaneurysmaversorgung (EVAR) und 3.923 Patienten mittels offener Versorgung (OAR) behandelt. Die Ergebnisse wurden zum Fallaufkommen der Kliniken in Beziehung gesetzt.
Ergebnisse: Weder für die Letalität noch für den MTL30-Wert konnten Volumen- Ergebnisbeziehungen gefunden werden. Bei EVAR lag die Sterblichkeit im Quintil 1 des Fallaufkommens bei 1,0% und im Quintil 5 bei 1,3%. Auch beim MTL30 ergaben sich keine signifikanten Unterschiede zwischen Quintil 1 und 5, der MTL30 belief sich jeweils auf 5,3%. Bei OAR war die Klinikletalität in dem Quintil mit dem geringsten Fallaufkommen (Quintil 1) mit 5,8% verglichen mit Quintil 5 (höchstes Fallaufkommen, Klinikletalität 3,5%) im Trend höher, aber nicht signifikant unterschiedlich (p= 0,505). Auch bei OAR korrelierte der MTL30 nicht mit dem Fallaufkommen (Quintil 1: 16,4% vs. Quintil 5:12,2%, p=0,110). Jedoch korrelierte die Liegedauer bei EVAR und OAR sowohl mit der Letalität (EVAR: Quintil 1 der Liegezeit 1,69% vs. Quintil 5 1,02%, p= 0,042; OAR: Quintil 1: 6,78% vs. Quintil 5: 2,16%, p= 0,000) als auch mit dem MTL30 (EVAR: Quintil 1 der Liegezeit 7,35% vs. Quintil 5 2,85%, p=0,000; OAR: Quintil 1 26,28% vs. Quintil 5 5,19%, p= 0,000).
Folgerung: Es konnte weder bei EVAR noch bei OAR ein Zusammenhang zwischen dem jährlichen Fallaufkommen und der Letalität bzw. dem MTL30 gefunden werden. Ob der MTL30 gegenüber der alleinigen Erfassung der Klinikletalität als Qualitätsparameter einen Zusatznutzen bietet, muss offenbleiben. Ein vielversprechenderer Marker scheint die stationäre Liegedauer zu sein, die zusätzlich zur Letalität als Qualitätsparameter verwendet werden könnte.
Fallzahlaufkommen und Qualitätsindikatoren bei der Versorgung des abdominellen Bauchaortenaneurysmas

Hintergrund: Der MTL30 (Mortalität, Transfer, Liegezeit) wurde als Surrogatparameter zur Evaluation der Qualität potenziell komplikationsträchtiger viszeralchirurgischer Eingriffe vorgeschlagen.
Zielsetzung: Es wurde überprüft, inwieweit sich der MTL30 zu den Ergebnissen des Bauchaortenaneurysma(AAA)-Registers des Deutschen Instituts für Gefäßmedizinische Gesundheitsforschung (DIGG) der Deutschen Gesellschaft für Gefäßchirurgie und Gefäßmedizin (DGG) und zum Fallaufkommen der Kliniken korrelieren lässt.
Material und Methoden: Insgesamt 14.282 Patienten wurden endovaskulär (EVAR) und 3923 Patienten offen (OAR) elektiv wegen eines AAA versorgt. Bestimmt wurden Fallaufkommen der behandelnden Kliniken, Klinikletalität, Liegezeit und Verlegung in ein anderes Akutkrankenhaus 30 Tage nach dem Indexeingriff.
Ergebnisse: Die Klinikletalität machte bei EVAR 1,3 %, bei OAR 4,9 % aus (p = 0,000), der MTL30 5,0 % vs. 14,4 % (p = 0,000). Für EVAR ließ sich keine Beziehung zwischen Fallaufkommen und Klinikletalität (Quintile 1: 1,0 %; Quintile 5: 1,3 %) sowie Fallaufkommen und MTL30 (Quintile 1: 5,3 %; Quintile 5: 5,3 %) nachweisen. Auch bei OAR bestand keine signifikante Beziehung zwischen Fallaufkommen und Klinikletalität (Quintile 1: 5,8 %, Quintile 5: 3,5 %; p = 0,505) und Fallaufkommen und MTL30 (Quintile 1: 16,4 %, Quintile 5: 12,2 %, p = 0,110). Bei einer Klinikletalität von 7,2 (5–10) % betrug der MTL30 bei OAR 17,6 %. Sowohl bei EVAR als auch bei OAR korrelierte die stationäre Aufenthaltsdauer signifikant mit Klinikletalität und MTL30.
Diskussion: Eine eindeutige Beziehung zwischen Krankenhausfallaufkommen und Klinikletalität ließ sich im AAA-Register des DIGG nicht aufzeigen. Das gleiche galt für den MTL30. Ob demnach der MTL30 gegenüber der Erfassung von Klinikletalität und stationärer Liegezeit als Qualitätsparameter einen Zusatznutzen bietet, muss offenbleiben.
Recommendations from the ESO-Karolinska Stroke Update Conference, Stockholm 13–15 November 2016

About the meeting: The purpose of the European Stroke Organisation (ESO)-Karolinska Stroke Update Conference is to provide updates on recent stroke therapy research and to give an opportunity for the participants to discuss how these results may be implemented into clinical routine. Several scientific sessions discussed in the meeting and each session produced consensus statements. The meeting started 20 years ago as Karolinska Stroke Update, but since 2014, it is a joint conference with ESO. Importantly, it provides a platform for discussion on the ESO guidelines process and on recommendations to the ESO guidelines committee on specific topics. By this, it adds a direct influence from stroke professionals otherwise not involved in committees and work groups on the guidelines procedure. The discussions at the conference may also inspire new guidelines when motivated. The topics raised at the meeting are selected by the scientific programme committee mainly based on recent important scientific publications. The ESO-Karolinska Stroke Update consensus statement and recommendations will be published every 2 years and it will work as implementation of ESO-guidelines
1 Purpose of the Study:
The purpose of this retrospective study was to assess the volumetric changes of our institutional pediatric neuroblastoma in response to various therapeutic protocols.
2 Materials and Methods:
A retrospective study was conducted on children with neuroblastoma from different anatomical locations including suprarenal, paraspinal, pelvic, mediastinal and cervical neuroblastoma primaries. These children underwent tumor-stage based therapeutic protocols in Johann Wolfgang Goethe University Hospital, Frankfurt am Main, Germany, between January 1996 and July 2008. The study included 72 patients (44 males and 28 females). Patient demographics (age and gender), disease-related symptoms, laboratory results (tumor biomarkers including ferritin, neuron specific enolase, and urine catecholamine) and histopathological reports were collected from the electronic medical archiving system and subsequently analyzed.
Patients were classified into following groups according the anatomical origin of the primary neuroblastoma into:
1) Suprarenal neuroblastoma Group: This group included patients with neuroblastoma arising from the suprarenal gland. This group composed of 54 patients with male to female ratio (32:22).
2) Paravertebral neuroblastoma Group: This group composed of 6 male patients.
3) Mediastinal neuroblastoma Group: This group included patients with mediastinal neuroblastoma and composed of 3 patients (1 male and 2 females).
4) Pelvic neuroblastoma Group: This group included patients with pelvic neuroblastoma and composed of 6 patients (3 males and 3 females).
5) Cervical neuroblastoma Group: This group included patients with cervical neuroblastoma and composed of 2 male patients.
3 Results:
The mean volume of all suprarenal neuroblastoma group involved in the study before therapy was 176.62 cm3 (SD: 234.15) range: 239.4-968.9cm3. The mean initial volume of all suprarenal neuroblastoma group who underwent observation protocol was 86.0378 cm3 (SD: 114.44) range: 5.2-347.94cm3. Volumetric evaluation of suprarenal neuroblastoma following observation (Wait and See) protocol revealed continuous reduction of the tumor volumes in a statistically significant manner during the follow up periods up to 12 months with p value of less than 0.05. The volumetric changes afterwards were statistically insignificant.
The mean initial volume of all suprarenal neuroblastoma group who underwent primary surgery protocol was 42.4 cm3 (SD: 28.5) range: 7.5-90cm3. Complete surgical resection of the tumor was not feasible in all lesions due to local tumor extension and / or infiltration with the associated risk of injury of nearby organs or structures. However statistical analysis of the volumetric changes in the successive follow up periods did not reveal statistical significance.
Volumetric estimation of the tumor in the subsequent follow up periods revealed significant changes within the period first (3-9 month periods). The changes afterwards were statistically non significant. On the other hand, the mean initial volume of all suprarenal neuroblastoma group who underwent combined chemotherapy and Stem cell transplantation protocol only without surgical interference was 99.98cm3 (SD:46.2) range: 48.48-160.48 cm3. In this group the volumetric changes were variable and difference in volumes in follow up was statistically non significant during the follow up period.
The mean initial volume of all abdominal paravertebral neuroblastoma group was 249.197cm3 (SD: 249.63) range: 9.6-934cm3. The mean initial volume of all pelvic neuroblastoma group was 118.88cm3 (SD: 50.61) range: 73.4-173.4cm3. The mean initial volume of all mediastinal neuroblastoma group was 189.7cm3 (SD: 139.057) range: 10.7-415 cm3. The mean initial volume of all cervical neuroblastoma group was 189.7cm3 (SD: 139.057) range: 10.7-415 cm3. The volumetric measurements in the corresponding follow up periods according to the therapeutic protocol of abdominal paravertebral neuroblastoma, pelvic neuroblastoma, mediastinal and cervical neuroblastoma revealed significant change in the tumor volume within the early 3-6 months from the initial therapy while subsequently the tumor volumetric changes were statistically non significant.
4 Conclusion:
In conclusion, the role of MRI volumetry in the evaluation of tumor response is dependent on the risk adapted concept of neuroblastoma with the combination of different imaging modalities as well the therapeutic protocol. MRI Volumetry in addition to new protocols such as Whole-body imaging and 3D visualization techniques are gaining more importance and acceptance.
Background: During the current second wave of COVID-19, the radiologists are expected to face great challenges in differentiation between COVID-19 and other virulent influenza viruses, mainly H1N1. Accordingly, this study was performed in order to find any differentiating CT criteria that would help during the expected clinical overlap during the current Influenza season.
Results: This study was retrospectively conducted during the period from June till November 2020, on acute symptomatic 130 patients with no history of previous pulmonary diseases; 65 patients had positive PCR for COVID-19 including 50 mild patients and 15 critical or severe patients; meanwhile, the other 65 patients had positive PCR for H1N1 including 50 mild patients and 15 critical or severe patients. They included 74 males and 56 females (56.9%:43.1%). Their age ranged 14–90 years (mean age 38.9 ± 20.3 SD). HRCT findings were analyzed by four expert consultant radiologists in consensus. All patients with COVID-19 showed parenchymal or alveolar HRCT findings; only one of them had associated airway involvement. Among the 65 patients with H1N1; 56 patients (86.2%) had parenchymal or alveolar HRCT findings while six patients (9.2%) presented only by HRCT signs of airway involvement and three patients (4.6%) had mixed parenchymal and airway involvement. Regarding HRCT findings of airway involvement (namely tree in bud nodules, air trapping, bronchial wall thickening, traction bronchiectasis, and mucous plugging), all showed significant p value (ranging from 0.008 to 0.04). On the other hand, HRCT findings of parenchymal or alveolar involvement (mainly ground glass opacities) showed no significant relation.
Conclusion: HRCT can help in differentiation between non-severe COVID-19 and H1N1 based on signs of airway involvement.
Einleitung: Frailty (engl. für Gebrechlichkeit) bezeichnet eine mit hohem Alter zunehmende Verschlechterung des körperlichen und kognitiven Zustandes von Individuen, woraufhin der Körper nicht mehr in der Lage ist, adäquat auf äußere und innere Stressoren zu reagieren. Frailty ist mit einer erhöhten Morbiditäts- und Mortalitätsrate sowie längerer Krankenhausverweildauer und erhöhter postoperativer Komplikationsrate verbunden und stellt folglich einen chirurgischen Risikofaktor dar.
Problemstellung: Die Relevanz eines strukturierten Frailty Assessments in der präoperativen Risikostratifizierung führte zur Indikationsstellung, diverse validierte Risk Assessment Tools auf ihre prädiktive Vorhersagekraft bezüglich des Auftretens von postoperativen Komplikationen und postoperativer Sterbewahrscheinlichkeit zu untersuchen.
Methoden: In die vorliegende Studie wurden Patienten, die in dem Zeitraum vom 01.09.2018 und 31.01.2019 in der allgemeinchirurgischen Ambulanz vorstellig waren und einen allgemeinchirurgischen Eingriff erhielten, aufgenommen. Mittels Fragebögen wurden die Scores „Risk Analysis Index“, „Edmonton Frail Scale“ sowie „Charlson Comorbidity Index“ präoperativ erhoben und retrospektiv mit Daten aus der digitalen Patientenakte zusammengeführt. Endpunkte waren die 90-Tages- Mortalität sowie das Auftreten von schweren postoperativen Komplikationen ab Clavien Dindo Grad 3b. Die Analyse erfolgte in SPSS mittels Chi-Quadrat Test, t- Test und ROC-Kurven Analysen.
Ergebnisse: Das durchschnittliche Alter der Studienkohorte lag bei 56 ± 15.9 Jahren und der Anteil männlicher Patienten überwog mit 59.2% (n=282).
Die Fragebögen wurden 739 Patienten vorlegt und 476 Patienten konnten in die Datenanalyse eingeschlossen werden. Die 90-Tages-Mortalität lag bei 2.7% (n=13) und 9% (n=43) erlitten schwere postoperative Komplikationen ab Clavien-Dindo Grad IIIb. Die Einteilung nach der ASA-Klassifikation (p=0.024), maligne Diagnosen -7-(p<0.001) und Majorkomplikationen (p<0.001) stellten präoperative Risikofaktoren für postoperative 90-Tage-Mortalität dar. Von den Risk Assessment Scores zeigte lediglich der Risk Analysis Index eine signifikante Korrelation auf (p=0.013). Ein mittels ROC-Analyse ermittelter Cut-Off Wert von 23 klassifizierte 166 (34.9%) Patienten als frail, die mit 69% Sensitivität und 66% Spezifität (AUC=0.735) ein erhöhtes Risiko für postoperatives Versterben innerhalb von 90 Tagen aufwiesen (p=0.008). Risikofaktoren für das Auftreten schwerer postoperativer Komplikationen waren die ASA-Klassifikation (p=0.041), längere Krankenhausverweildauer (p<0.001) und maligne Diagnosen (p<0.001). Der Charlson Comorbidity Index (p=0.031) und RAI-C Werte ≥ 23 (p<0.001) korrelierten signifikant mit Majorkomplikationen. Das Alter ab 65 Jahren stellte mit 77 % Spezifität und 69 % Sensitivität ebenfalls einen prädiktiven Risikofaktor für postoperative Mortalität dar (AUC=0,787).
Schlussfolgerung: Mithilfe validierter Risk Assessment Tools ist es möglich Patienten, die ein erhöhtes Risiko für postoperative negative Ereignisse aufweisen, bereits präoperativ zu erkennen. Dies ermöglicht eine bessere Beurteilung der chirurgischen Indikationsstellung sowie das rechtzeitige Ergreifen von risikominimierenden Maßnahmen. Es ist notwendig die Ergebnisse dieser Arbeit künftig mit risikominimierenden Maßnahmen zu verknüpfen und zu untersuchen, ob die Implementierung der Risk Assessment Tools zu verbesserten postoperativen Ergebnissen führt, wenn modifizierbare Faktoren verbessert werden.
In dieser klinischen, randomisierten, doppelverblindeten Vergleichsstudie untersuchten wir, welchen Einfluss auf die Passgenauigkeit von Einzelzahnkronen und dreigliedrigen Brücken aus Vollkeramik zum einen die Anwendung einer intraoralen Digitalisierung mithilfe eines Intraoralscanners (Lava C.O.S Scanner, 3M ESPE, D-Seefeld) und zum anderen der Einsatz einer extraoralen Digitalisierung auf Basis einer konventionellen Abformung (Impregum penta soft, 3M ESPE, D-Seefeld) hat. Als weiteres Prüfkriterium wurde die Effizienz der jeweiligen Abformmethoden untersucht. Die Fragestellung sollte eine Aussage dazu treffen können, ob die digitale Abformung im klinischen Alltag des Zahnarztes einen Vorteil gegenüber der konventionellen Abformung erbringen kann.
In der Studie wurden 25 Patienten eingeschlossen und beide Abformmethoden nach der Zahnpräparation in einer randomisierten Abfolge angewandt. Beim Intraoralscanner waren ein Ganzkieferscan für die Brückenrestaurationen und ein Quadrantenscan für Einzelzahnkronen erforderlich. Die klinischen Arbeitsschritte wurden zeitlich erfasst. Die Kronen- und Brückengerüste aus Zirkoniumdioxidkeramik basierten auf den unterschiedlichen Abformmethoden. Die Gerüste wurden vor der Anprobe verblindet und mit Hilfe von Silikonreplikas die Passgenauigkeit am marginalen Randspalt, an der axialen Wand, am axio-okklusalen Übergang, sowie am okklusalen Messpunkt unter 66x Vergrößerung ermittelt.
Der marginale Randspalt der Restaurationen auf der Basis der konventionellen Abformung betrug 68,64 μm (Medianwert), respektive 60,31 μm bei der digitalen Abformung und unterschied sich statistisch signifikant. Die Passung an der axialen Wand ergab bei der digitalen Abformung 88,27 μm, bei konventioneller 92,13 μm, am axio-okklusalen Übergang 144,78 μm vs. 155,60 μm, am okklusalen Messpunkt 155,57 μm vs. 171,51 μm. Letzter wies einen statistisch signifikanten Unterschied auf (Mann-Whitney U Test, p = 0,05). Die Zeitmessung zeigte eine Ersparnis von 5 min 6 sec beim Quadrantenscan und 1 min 34 sec beim Ganzkieferscan zu Gunsten der digitalen Abformung.
Die Studienergebnisse weisen eine effizientere klinische Arbeitsweise der digitalen gegenüber der konventionellen Abformung nach, die mit einer identischen Passungsqualität einhergeht.
The COVID-19 pandemic and resulting measures can be regarded as a global stressor. Cross-sectional studies showed rather negative impacts on people’s mental health, while longitudinal studies considering pre-lockdown data are still scarce. The present study investigated the impact of COVID-19 related lockdown measures in a longitudinal German sample, assessed since 2017. During lockdown, 523 participants completed additional weekly online questionnaires on e.g., mental health, COVID-19-related and general stressor exposure. Predictors for and distinct trajectories of mental health outcomes were determined, using multilevel models and latent growth mixture models, respectively. Positive pandemic appraisal, social support, and adaptive cognitive emotion regulation were positively, whereas perceived stress, daily hassles, and feeling lonely negatively related to mental health outcomes in the entire sample. Three subgroups (“recovered,” 9.0%; “resilient,” 82.6%; “delayed dysfunction,” 8.4%) with different mental health responses to initial lockdown measures were identified. Subgroups differed in perceived stress and COVID-19-specific positive appraisal. Although most participants remained mentally healthy, as observed in the resilient group, we also observed inter-individual differences. Participants’ psychological state deteriorated over time in the delayed dysfunction group, putting them at risk for mental disorder development. Consequently, health services should especially identify and allocate resources to vulnerable individuals.
Background Endothelium-derived nitric oxide plays an important role for the bone marrow microenvironment. Since several important effects of nitric oxide are mediated by cGMP-dependent pathways, we investigated the role of the cGMP downstream effector cGMP-dependent protein kinase I (cGKI) on postnatal neovascularization. Methodology/Principal Findings In a disc neovascularization model, cGKI -/- mice showed an impaired neovascularization as compared to their wild-type (WT) littermates. Infusion of WT, but not cGKI -/- bone marrow progenitors rescued the impaired ingrowth of new vessels in cGKI-deficient mice. Bone marrow progenitors from cGKI -/- mice showed reduced proliferation and survival rates. In addition, we used cGKI alpha leucine zipper mutant (LZM) mice as model for cGKI deficiency. LZM mice harbor a mutation in the cGKI alpha leucine zipper that prevents interaction with downstream signaling molecules. Consistently, LZM mice exhibited reduced numbers of vasculogenic progenitors and impaired neovascularization following hindlimb ischemia compared to WT mice. Conclusions/Significance Our findings demonstrate that the cGMP-cGKI pathway is critical for postnatal neovascularization and establish a new role for cGKI in vasculogenesis, which is mediated by bone marrow-derived progenitors.

Abstract
Immune dysregulation has been reported in schizophrenia spectrum disorders (SSD). In the past decade, several trials using anti-inflammatory agents for treatment of SSD have been completed, with so far limited success. One such anti-inflammatory agent used is simvastatin. A recent, large-scale, randomized controlled trial with simvastatin augmentation failed to show improvement in the predefined primary outcome. However, baseline inflammatory profiles were not taken into account. Here we employed a data-driven clustering approach to investigate whether patients with an inflammatory monocyte gene signature respond better to add-on simvastatin treatment than those without such a signature, over a treatment period of 2 years. In 61 patients (60 randomized, 1:1 placebo:simvastatin) and healthy controls, a previously validated monocyte gene expression signature was assessed using quantitative polymerase chain reaction. Resulting delta cycle threshold values were used to identify patient clusters. Two major patient clusters with either up- or downregulated pro-inflammatory factors were detected. Linear mixed models showed a significant three-way interaction between the inflammatory cluster, treatment, and time for psychotic symptoms. Only patients treated with simvastatin who were in the inflammatory group, showed a consistent improvement: symptom severity gradually decreased after 3 months and reached significance after 12 and 24 months compared to baseline (p.adj<0.05). The effects were small, and overall between-group effects were not significant. Here, we show that patient stratification based on inflammatory gene expression might be useful to select appropriate treatment augmentation for patients with SSD, highlighting the need for precision medicine approaches. Our findings corroborate the results of the primary analyses, showing that in the overall group, simvastatin was not effective; however, at the individual level the treatment might make a difference.
Background: The COVID-19 pandemic led to a higher incidence of depression and a worsening of psychiatric conditions, while pre-existing constraints of the healthcare system and safety regulations limited psychiatric care.
Aims: We investigated the impact of the pandemic on the clinical care of patients with a single episode (SE-MDD) or major depressive disorder (MDD) in Germany.
Methods: Nationwide inpatient data were extracted from the German Institute for Hospital Remuneration System for 2020 and 2021 (depression data) and the Robert Koch Institute (COVID-19 incidence). Changes in inpatients were tested with linear regression models. Local cases of depression in our department compared to 2019 were explored with one-way ANOVA and Dunnett's test.
Results: Across Germany, the inpatient numbers with both SE-MDD and MDD declined by more than 50% during three out of four COVID-19 waves. Higher COVID-19 incidence correlated with decreased inpatient numbers. In our department, fewer MDD inpatients were treated in 2020 (adj. p < 0.001) and 2021 (adj. p < 0.001) compared to 2019, while the number of SE-MDD inpatients remained stable. During this period fewer elective and more emergency inpatients were admitted. In parallel, MDD outpatient admissions increased in 2021 compared to 2019 (adj. p = 0.002) and 2020 (adj. p = 0.003).
Conclusion: During high COVID-19 infection rates, MDD patients received less inpatient care, which might cause poor outcomes in the near future. These data highlight the necessity for improved infrastructure in the in- and outpatient domains to facilitate accessibility to adequate care.
In the complex network of nuclear hormone receptors, the long non-coding RNAs (lncRNAs) are emerging as critical determinants of hormone action. Here we investigated the involvement of selected cancer-associated lncRNAs in Estrogen Receptor (ER) signaling. Prior studies by Chromatin Immunoprecipitation (ChIP) Sequencing showed that in prostate cancer cells ERs form a complex with the endothelial nitric oxide synthase (eNOS) and that in turn these complexes associate with chromatin in an estrogen-dependent fashion. Among these associations (peaks) we focused our attention on those proximal to the regulatory region of HOTAIR and MALAT1. These transcripts appeared regulated by estrogens and able to control ERs function by interacting with ERα/ERβ as indicated by RNA-ChIP. Further studies performed by ChIRP revealed that in unstimulated condition, HOTAIR and MALAT1 were present on pS2, hTERT and HOTAIR promoters at the ERE/eNOS peaks. Interestingly, upon treatment with17β-estradiol HOTAIR recruitment to chromatin increased significantly while that of MALAT1 was reduced, suggesting an opposite regulation and function for these lncRNAs. Similar results were obtained in cells and in an ex vivo prostate organotypic slice cultures. Overall, our data provide evidence of a crosstalk between lncRNAs, estrogens and estrogen receptors in prostate cancer with important consequences on gene expression regulation.
Methodik: Die in dieser Arbeit entwickelte flowzytometrische Methode ist aufgrund der guten Korrelation zur gängigen Europium Immune Release Methode zur Bestimmung der NK-Zell-, bzw. CD 8 , zytotoxischen T-Lymphozyten Aktivität gut geeignet. Der geringe Zeit- und Kostenaufwand sprechen für die Anwendung dieser Methode. Enzympräparation: Die in dieser Arbeit verwendete Enzympräparation kann die NK-Zell bzw. CD 8 , zytotoxischen T-Lymphozyten Aktivität signifikant steigern. Eine signifikante Mehrfachstimulation ist ebenfalls möglich. Möglicherweise beruht dieser Effekt auf einer erhöhten Freisetzung von TNFa durch die Effektorzellen, die die Targetzellen zum (programmierten) Zelltod bringen. Ferner wird über die Freisetzung weiterer Zytokine berichtet, die eine Stimulierung der Effektorzellen bewirken (durch IL1b und IL6). Immunkomplex gebundene TNFa Moleküle können durch die proteolytische Aktivität freigesetzt werden. Die Ergebnisse weisen auf eine unspezifische Veränderung der Membranbestandteile der Effektorzellen hin, welche eine Freisetzung von Zytokinen bewirkt. Ferner kann die proteolytische Aktivität der Enzympräparate verschiedene Immunkomplexe und sogenannte Blockingfaktoren auflösen. Fazit: Eine signifikante und konzentrationsabhängig immunstimulierende Wirksamkeit der Enzyme ist gegeben. Diskussionswürdig dürfte hier nur die enterale Resorption bzw. Persorption sein (s.Kap I). Die parenterale Applikation ist wegen einer anaphylaktischen Reaktion risikoreich. Denkbar wäre eine ex vivo "Therapie" isolierter humaner Lymphozyten mit anschließender Wiederzufuhr in den Kreislauf. Die Aktivierenden Konzentrationen für die Enzyme liegen im Bereich von 20 bis 160 µg/ml bei einer Einfachstimulation. Bei höheren Konzentrationen als 320 µg/ml beginnt der toxische Bereich. Bei der Mehrfachstimulation von Lymphozyten liegt der aktivierende Bereich zwischen 10 bis 80 µg/ml, wobei die höheren Konzentrationen toxisch wirken. Retinoide: Eine konzentrationsabhängige signifikante Steigerung der NK-Zell- bzw. CD 8 , zytotoxischen T-Lymphozyten Aktivität ist gegeben. Die NK Zellaktivität konnte bei Konzentrationen von 10 8 bzw. 10 6 M mit Retinsäure und Retinol gesteigert werden, die Aktivität von CD 8 zytotoxischen T-Lymphozyten hingegen nur durch Retinsäure. Das Lösungsmittel DMSO bewirkte keine signifikante Aktivitätsänderungen. Diskutiert wird hier eine erhöhte Expression des IL2-Rezeptors sowie die des CD3 Rezeptors, als Ursache für die Aktivitätssteigerung führen soll. Der Fas-Ligand hingegen wird herabreguliert, was eine "Selbsttötung" der Effektorzellen verhindert. Die vermehrte Produktion von Zytokinen (IL2, IL4, IL6, IL10 und IFNgamma), deren Gehalt die im Überstand Retinol behandelter Zellen zunimmt, kann ebenfalls zur Aktivitätssteigerung führen.
Excessive accumulation of the extracellular matrix is a hallmark of many inflammatory and fibrotic diseases, including those of the kidney. This study addresses the question whether NO, in addition to inhibiting the expression of MMP-9, a prominent metalloprotease expressed by mesangial cells, additionally modulates expression of its endogenous inhibitor TIMP-1. We demonstrate that exogenous NO has no modulatory effect on the extracellular TIMP-1 content but strongly amplifies the early increase in cytokine-induced TIMP-1 mRNA and protein levels. We examined whether transforming growth factor beta (TGFbeta), a potent profibrotic cytokine, is involved in the regulation of NO-dependent TIMP-1 expression. Experiments utilizing a pan-specific neutralizing TGFbeta antibody demonstrate that the NO-induced amplification of TIMP-1 is mediated by extracellular TGFbeta. Mechanistically, NO causes a rapid increase in Smad-2 phosphorylation, which is abrogated by the addition of neutralizing TGFbeta antisera. Similarly, the NO-dependent increase in Smad-2 phosphorylation is prevented in the presence of an inhibitor of TGFbeta-RI kinase, indicating that the NO-dependent activation of Smad-2 occurs via the TGFbeta-type I receptor. Furthermore, activation of the Smad signaling cascade by NO is corroborated by the NO-dependent increase in nuclear Smad-4 level and is paralleled by increased DNA binding of Smad-2/3 containing complexes to a TIMP-1-specific Smad-binding element (SBE). Reporter gene assays revealed that NO activates a 0.6-kb TIMP-1 gene promoter fragment as well as a TGFbeta-inducible and SBE-driven control promoter. Chromatin immunoprecipitation analysis also demonstrated DNA binding activity of Smad-3 and Smad-4 proteins to the TIMP-1-specific SBE. Finally, by enzyme-linked immunosorbent assay, we demonstrated that NO causes a rapid increase in TGFbeta(1) levels in cell supernatants. Together, these experiments demonstrate that NO by induction of the Smad signaling pathway modulates TIMP-1 expression.
Hintergrund: Die Endometriose und die Infertilität sind beide Erkrankungsbilder, die häufig miteinander assoziiert sind. So werden Literaturen zur Folge bei mindestens 30% der Endometriose Patientinnen eine Sterilität diagnostiziert, während wiederum bei etwa 25 - 50% der Patientinnen eine Endometriose als Hauptursache des unerfüllten Kinderwunsches erkannt wird. Die Laparoskopie dient bei Patientinnen mit Endometriose als wichtiger invasiver Eingriff, der entweder zu diagnostischen Zwecken im Sinne einer Ursachenabklärung dient und / oder zu therapeutischen Zwecken bei bereits bestätigter Endometriose assoziierter Infertilität durchgeführt wird.
Problemstellung und Zielsetzung: Anhand des laparoskopischen Befundes, z.B. durch das Erfassen des rASRM Scores bei Endometriose, kann Patientinnen im Voraus keine zuverlässige Aussage über die Chance auf eine postoperative Schwangerschaft geäußert werden. Das Ziel der hier vorliegenden Arbeit ist es, den Erfolg der laparoskopischen Behandlung bei Endometriose assoziierter Infertilität unter Einbezug von Patientencharakteristika, des intraoperativen Befundes, operativer Verfahren sowie der postoperativen Fertilitätsbehandlung zu beurteilen. Auf diese Weise soll den Frauen geholfen werden, im Voraus bzw. nach laparoskopischer Behandlung der Endometriose die Chance auf eine postoperative Schwangerschaft besser abzuschätzen. Es sollen somit Faktoren ermittelt werden, die die postoperative Fertilität protektiv oder ungünstig beeinflussen.
Material und Methoden: In die Studie aufgenommen wurden alle Patientinnen (n=102), die im Zeitraum von 2007 bis 2017 an der Universitätsklinik Frankfurt am Main einen laparoskopischen Eingriff mit der Diagnose einer Endometriose erhalten haben und einen unerfüllten Kinderwunsch von mehr gleich einem Jahr äußerten. Die Datenerhebung erfolgte über die elektronische Patientenakte sowie die telefonische Befragung der Studienteilnehmerinnen zur Patientencharakteristika und der postoperativen Fertilitätsbehandlung. Die statistische Auswertung wurde durch die Nutzung des Log Rank Tests / der Kaplan Meier Methode sowie uni- und multivariater Analysen mittels
Cox Regression durchgeführt.
Ergebnisse: Von den 102 Patientinnen konnten nach einer mittleren postoperativen Nachbeobachtungszeit von 70 Monaten 71 Patientinnen (69,6%) eine Schwangerschaft verwirklichen. Dabei wurden 21,6% der Frauen durch die alleinige Laparoskopie schwanger, während bei 48% der Frauen die Schwangerschaft durch die zusätzliche Nutzung reproduktionsmedizinischer Verfahren erreicht wurde. Die meisten Schwangerschaften ereigneten sich in den ersten 6 bis 12 Monaten nach der Operation. Die mittlere Dauer bis zur Schwangerschaft betrug 10 Monate. Der Log Rank Test zeigte keine statistisch signifikanten Unterschiede in den verschiedenen rASRM Stadien bei der Betrachtung der kumulativen Schwangerschaftsrate (rASRM 1: 83,3%; rASRM 2: 66,7%; rASRM 3: 93,8%; rASRM 4: 37,9%). Die uni- und multivariaten Analysen zeigten, dass die Dauer der Infertilität (Hazard Ratio: 0,85), das Vorkommen von intestinaler Endometriose (Hazard Ratio: 0,26) und ausgeprägten Adhäsionen (Hazard Ratio: 0,45), die Anzahl der Endometriose Lokalisationen (Hazard Ratio:0,82), die Anzahl der laparoskopischen Eingriffe für Endometriose (Hazard Ratio: 0,66) sowie der Voroperationen (Hazard Ratio: 0,75) und das Vorkommen von Endometriose Rezidiven (Hazard Ratio: 0,4) die Chance auf eine Konzeption signifikant ungünstig beeinträchtigen (p< 0,05). Ein protektiver Einfluss auf die Schwangerschaft zeigte sich nur für die Koagulation am Ovar (p= 0,02; Hazard Ratio: 7,74).
Schlussfolgerung: Die laparoskopische Behandlung scheint die Erfüllung des Kinderwunsches positiv zu beeinflussen, zumal fast ¾ der Patientinnen postoperativ schwanger wurden. Höhere Schwangerschaftsraten wurden durch die zusätzliche Nutzung der assistierten Reproduktionstherapie (ART) erreicht. Aus diesem Grund sollten Frauen für eine zusätzliche postoperative ART motiviert werden. Die Patientinnen sollten darauf aufmerksam gemacht werden, dass sie sich bei einem unerfüllten Kinderwunsch früher bei ihrem Arzt vorstellen sollten. Denn je länger die Zeit bis zur Operation einer Endometriose assoziierten Infertilität beträgt, desto schlechter ist die Chance auf eine postoperative Schwangerschaft (p= 0,02). Endometriosezysten am Ovar sowie deren Entfernung schränken die klinische Fertilität nicht signifikant ein. Die Anzahl der laparoskopischen Eingriffe sowie der abdominellen Voroperationen scheinen ein wichtigerer, ungünstiger Faktor in Hinblick auf die Schwangerschaft zu sein (p = 0,04). Außerdem könnte die Koagulation am Ovar eine wichtige Behandlungsmethode bei Endometriose assoziierter Infertilität sein. Diese Operationsmethode sollte durch weitere Studien und eine größere Stichprobe untersucht werden...
Iron deficiency, with or without anemia, is the most frequent hematological manifestation in individuals with cancer, and is especially common in patients with colorectal cancer. Iron is a vital micronutrient that plays an essential role in many biological functions, in the context of which it has been found to be intimately linked to cancer biology. To date, however, whereas a large number of studies have comprehensively investigated and reviewed the effects of excess iron on cancer initiation and progression, potential interrelations of iron deficiency with cancer have been largely neglected and are not well-defined. Emerging evidence indicates that reduced iron intake and low systemic iron levels are associated with the pathogenesis of colorectal cancer, suggesting that optimal iron intake must be carefully balanced to avoid both iron deficiency and iron excess. Since iron is vital in the maintenance of immunological functions, insufficient iron availability may enhance oncogenicity by impairing immunosurveillance for neoplastic changes and potentially altering the tumor immune microenvironment. Data from clinical studies support these concepts, showing that iron deficiency is associated with inferior outcomes and reduced response to therapy in patients with colorectal cancer. Here, we elucidate cancer-related effects of iron deficiency, examine preclinical and clinical evidence of its role in tumorigenesis, cancer progression and treatment response. and highlight the importance of adequate iron supplementation to limit these outcomes.
Chronic intestinal failure (CIF) is a rare but feared complication of Crohn’s disease. Depending on the remaining length of the small intestine, the affected intestinal segment, and the residual bowel function, CIF can result in a wide spectrum of symptoms, from single micronutrient malabsorption to complete intestinal failure. Management of CIF has improved significantly in recent years. Advances in home-based parenteral nutrition, in particular, have translated into increased survival and improved quality of life. Nevertheless, 60% of patients are permanently reliant on parenteral nutrition. Encouraging results with new drugs such as teduglutide have added a new dimension to CIF therapy. The outcomes of patients with CIF could be greatly improved by more effective prevention, understanding, and treatment. In complex cases, the care of patients with CIF requires a multidisciplinary approach involving not only physicians but also dietitians and nurses to provide optimal intestinal rehabilitation, nutritional support, and an improved quality of life. Here, we summarize current literature on CIF and short bowel syndrome, encompassing epidemiology, pathophysiology, and advances in surgical and medical management, and elucidate advances in the understanding and therapy of CIF-related complications such as catheter-related bloodstream infections and intestinal failure-associated liver disease.
Evidence gained from recent studies has generated increasing interest in the role of vitamin D in extraskeletal functions such as inflammation and immunoregulation. Although vitamin D deficiency has been implicated in the pathophysiology of inflammatory diseases including inflammatory bowel disease (IBD), evidence as to whether vitamin D supplementation may cure or prevent chronic disease is inconsistent. Since 25OH-vitamin D (25OHD) has been suggested to be an acute-phase protein, its utility as a vitamin D status marker is therefore questionable. In this study, possible interactions of vitamin D and inflammation were studied in 188 patients with IBD, with high-sensitivity C-reactive protein (hsCRP) levels ≥ 5 mg/dL and/or fecal calprotectin ≥ 250 µg/g defined as biochemical evidence of inflammatory activity. Levels of 25OHD and vitamin D-binding protein (VDBP) were determined by ELISA, and 1,25-dihydroxyvitamin D (1,25OHD) and dihydroxycholecalciferol (24,25OHD) by LC-MS/MS. Free and bioavailable vitamin D levels were calculated with the validated formula of Bikle. Serum 1,25OH2D and vitamin D binding protein (VDBP) levels were shown to differ between the inflammatory and noninflammatory groups: patients with inflammatory disease activity had significantly higher serum concentrations of 1,25OH2D (35.0 (16.4–67.3) vs. 18.5 (1.2–51.0) pg/mL, p < 0.001) and VDBP (351.2 (252.2–530.6) vs. 330.8 (183.5–560.3) mg/dL, p < 0.05) than patients without active inflammation. Serum 24,25OH2D levels were negatively correlated with erythrocyte sedimentation rate (ESR) (−0.155, p = 0.049) while concentrations of serum 1,25OH2D correlated positively with hsCRP (0.157, p = 0.036). Correlations with serum VDBP levels were found for ESR (0.150, p = 0.049), transferrin (0.160, p = 0.037) and hsCRP (0.261, p < 0.001). Levels of serum free and bioavailable 25OHD showed a negative correlation with ESR (−0.165, p = 0.031, −0.205, p < 0.001, respectively) and hsCRP (−0.164, p = 0.032, −0.208, p < 0.001 respectively), and a moderate negative correlation with fecal calprotectin (−0.377, p = 0.028, −0.409, p < 0.016, respectively). Serum total 25OHD concentration was the only vitamin D parameter found to have no specific correlation with any of the inflammatory markers. According to these results, the traditional parameter, total 25OHD, still appears to be the best marker of vitamin D status in patients with inflammatory bowel disease regardless of the presence of inflammation.
Nach Eickholz (2005) ist die bakterielle Plaque die notwendige, wenn auch nicht die alleinige Ursache für die Entwicklung der entzündlichen Zerstörung des Zahnhalteapparats bei Parodontitis. Die Plaque-Bakterien geben Stoffwechselprodukte ab, die zur Reizung des Zahnfleisches bis hin zur Zahnfleischentzündung mit einhergehenden Blutungen und der Bildung von Zahnfleischtaschen führen. Jeder Parodontitis geht immer eine Gingivitis voraus, während nicht jede Gingivitis zu einer Parodontitis führen muss (Löe et al. 1986). Der Entzündungsprozess kann sich auf den gesamten Zahnhalteapparat ausbreiten und letztendlich zum Zahnverlust führen (GABA 2005). Die Parodontalbehandlung stützt sich auf die Beseitigung oder zumindest auf die Verringerung der pathogenen Mikroorganismen. Dies erfolgt durch Kürettage und Wurzelglättung der Wurzeloberfläche. Die Effektivität der Parodontalbehandlung ist jedoch nur dann gesichert, wenn während der Nachsorgephase eine Patienten-Compliance gesichert ist. In der vorliegenden Studie werden relevante Daten aus den Patientendaten der Poliklinik für Parodontologie in Frankfurt am Main entnommen und zusammen mit den Daten einer aktuellen Untersuchung analysiert. Es sollte der Therapieerfolg in Abhängigkeit zur Compliance des Patienten bewertet werden. Die Ergebnisse der vorliegenden Studie lassen sich wie folgt zusammenfassen: Zur Nachuntersuchung hatten sich 76 Patienten (46 Frauen und 30 Männer) mit einem durchschnittlichen Alter von 63,5 Jahren zur aktuellen Untersuchung erneut vorgestellt. Diese Patienten waren vor 10 – 21 Jahren am Carolinum einer systematischer Parodontalbehandlung unterzogen worden. 32 Patienten (Testgruppe) hatten danach regelmäßig (1 - 2 x jährlich) am Recall teilgenommen, während 44 Patienten (Kontrollgruppe) nicht regelmäßig zum Recall erschienen waren. 12 der 76 Patienten waren Raucher. Als klinische Parameter dienten Zahnverlust, Approximalraum-Plaque-Index (API), Taschensondierungstiefe, Blutung nach Sondierung (BOP), sondierbare Furkationen und gingivale Rezessionen. Die Resultate ergeben, dass bei der Testgruppe 2,13 Zähne und bei der Kontrollgruppe 1,3 Zähne seit Therapiebeginn verloren wurden. Der durchschnittliche Zahnverlust aller Patienten betrug 1,65 Zähne. Betrachten wir nur die Raucher, so haben diese 3,08 Zähne pro Patient im Durchschnitt verloren. Der durchschnittliche API- Wert lag vor der Behandlung bei der Testgruppe bei 25%, nach 1 Jahr bei 31,9% und beim aktuellen Befund bei 41%. Die Kontrollgruppe hatte vor der Behandlung einen API von 24,6%, nach 1 Jahr 50,4% und beim aktuellen Befund 50,3%. Die durchschnittliche Sondierungstiefe wurde bei der Testgruppe ein Jahr nach Behandlung um 0,37 mm und bei der aktuellen Untersuchung um 0,87 mm reduziert. Bei der Kontrollgruppe wurde die durchschnittliche Sondierungstiefe nach dem ersten Jahr um 0,26 mm und zum aktuellen Befund um 0,34 mm gesenkt. Von Bedeutung ist die Beurteilung der Blutung nach Sondierung, welcher den Entzündungsgrad des Zahnhalteapparats widerspiegelt. Patienten der Testgruppe hatten im ersten Jahr nach Behandlung ihren durchschnittlichen BOP um 12,3% und zur aktuellen Untersuchung um weitere 10% auf 21,7% reduziert. Bei der Kontrollgruppe wurde der durchschnittliche BOP im ersten Jahr um 10,1% und zur aktuellen Untersuchung um weitere 5,5% auf 25,5% reduziert. Die sondierbaren Furkationen haben in beiden Gruppen zugenommen. Beim aktuellen Befund waren im Durchschnitt bei der Testgruppe 2,8 und bei der Kontrollgruppe 2,6 Parodontien sondierbar. Hinsichtlich der Rezessionen gab es bei der Test- und Kontrollgruppenpatienten keine Unterschiede. In beiden Gruppen hatte die Rezession im Durchschnitt auf einen Wert von 1 mm behandlungsbedingt zugenommen. Mit dieser Untersuchung konnte belegt werden, dass bei der Testgruppe ein besserer parodontaler Zustand (API, Taschentiefen, BOP) nach einer systematischen Parodontalbehandlung erzielt und erhalten werden konnte, als es bei der Kontrollgruppe, die nicht regelmäßig zum Recall erschien, der Fall war. Demnach kann dem am Carolinum praktizierten Recallsystem ein gewisser günstiger Einfluss auf die Zahn- und Parodontalgesundheit zugeschrieben werden. Man hätte sich Daten gewünscht, die diesen Effekt noch stärker untermauern. Es ist jedoch zu bedenken, dass alle Recall-Termine nur Momentaufnahmen des Patienten ergeben, die nichts darüber aussagen können, wie mundgesundheitsbewusst sich dieser Patient in der monatelangen Zeit zwischen seinen Zahnarztterminen verhält. Solange keine besseren Möglichkeiten der Einflussnahme seitens des Zahnarztes bestehen, muss deshalb das Recall-System mit seinen nachweisbaren (wenn auch bescheidenen) Erfolgen, als praktikable Methode hin zur besseren oralen Gesundheit angesehen werden.
Acute posterior multifocal placoid pigment epitheliopathy (APMPPE) is an uncommon disorder of unknown etiology affecting the retina, the retinal pigment epithelium, and the choroid. Although several etiological factors have been suggested, none has been confirmed. We report a case of APMPPE associated with acute infection of Borreliosis. A 30-year-old man presented with a decrease in vision in the right eye of about 1-week duration. His visual acuity in the right eye was 6/36. Fundus exam revealed the presence of multiple placoid creamy retinal/subretinal lesions in the right eye. Fundus fluorescein angiography supported the diagnosis of APMPPE. Blood tests revealed the presence of concomitant acute Borreliosis infection, as confirmed by IgM. The patient received oral prednisone therapy and amoxicillin. Six weeks later, the visual acuity returned to 6/6, and the patient was symptom free. Borreliosis can have several manifestations in the eye. One of the less common presentations is an APMPPE-like picture. The clinician should suspect acute Borreliosis infection in patients presenting with APMPPE, especially when there is a history of a tick bite, when the patient has systemic symptoms, or when living in/visiting endemic areas. This may help in the prompt management of APMPPE, avoiding complications due to the condition itself, or systemic involvement secondary to the Borreliosis infection.
Objective: To assess the effect of cesarean section (CS) timing, elective versus unplanned, on the residual myometrial thickness (RMT) and CS scars. Methods: This is a prospective single-blinded observational cohort study with 186 observations. Patients indicated to undergo first singleton CS were preoperatively recruited. Exclusion criteria were history of repeated CS, vertical hysterotomy, diabetes, and additional uterine surgeries. Sonographic examination was performed for assessing the RMT ratio, the presence of a niche, fibrosis, and the distance from the scar to the internal os (SO) 1 year after CS. Power analysis was performed with 0.05 α, 0.1 β, and all statistical analyses were conducted with Stata®. Results: Wilcoxon rank-sum test for the association between CS timing, RMT ratio and SO showed Z values of −0.59 and −4.94 (P = 0.553 and P < 0.001), respectively. There was no association between CS timing and niches and fibrosis (P > 0.99 and P = 0.268, respectively). Linear regression between SO and the extent of cervical dilatation showed a −0.45 β (95% confidence interval −0.68 to −0.21) and a 10.22-mm intercept (P < 0.001). Conclusion: RMT is independent of the timing of CS, but the SO distance shows a negative linear relationship with the cervical dilatation.

Purpose: The aim of this study was to investigate characteristics associated with ectopic pregnancy (EP) that could be utilized for predicting morbidity or mortality.
Methods: This was a retrospective analysis of pregnancy-related records from a tertiary center over a period of ten years. Data on age, gravidity, parity, EP risk, amenorrhea duration, abdominal pain presence and location, β-human chorionic gonadotropin (β-HCG) level, ultrasound findings, therapeutic intervention, exact EP implantation site and length of hospital stay (LOS) were obtained from the database. The LOS was used as a proxy for morbidity and was tested for an association with all variables. All statistical analyses were conducted with Stata® (ver. 16.1, Texas, USA).
Results: The incidence of EP in a cohort of 30,247 pregnancies over a ten-year period was 1.05%. Patients presented with lower abdominal pain in 87.9% of cases, and the likelihood of experiencing pain was tenfold higher if fluid was detectable in the pouch of Douglas. Only 5.1% of patients had a detectable embryonic heartbeat, and 18.15% had one or more risk factors for EP. While most EPs were tubal, 2% were ovarian. The LOS was 1.9 days, and laparoscopic intervention was the main management procedure. The cohort included one genetically proven dizygotic heterotopic pregnancy (incidence, 3.3 × 10− 5) that was diagnosed in the 7th gestational week. The only association found was between the β-HCG level and LOS, with a linear regression β coefficient of 0.01 and a P-value of 0.04.
Conclusion: EP is a relatively common condition affecting approximately 1% of all pregnancies. β-HCG correlates with EP-related morbidity, but the overall morbidity rate of EP is low regardless of the implantation site. Laparoscopic surgery is an effective therapeutic procedure that is safe for managing EP, even in cases of heterotopic pregnancy.
Purpose: The aim of this study is to utilize the Manchester scar scale (MSS) and ultrasound in investigating the association between uterine wall defects and cutaneous scar characteristics after cesarean section (CS).
Methods: This is a prospective cohort study. The degree of myometrial loss was quantified by calculating a residual myometrial thickness (RMT) ratio as a percentage of RMT to the pre-cesarean anterior uterine wall thickness. Cutaneous scar assessment was performed according to the MSS. Spearman’s correlation and the Kruskal–Wallis test with a cut-off value of p < 0.05 were used for statistical analysis.
Results: Two hundred forty seven women, of which 2.4% had an Asian, 3.6% an Afro-American, 82% a Caucasian and 12% a Mediterranean background, were recruited. The RMT ratio ranged between 11.9 and 100% with a median of 55.8% and an average of 56%. MSS scores ranged from 4 to 13 with a median of 5 and an average of 6. Spearman’s correlation between MSS and RMT ratio show a rho of − 0.01 with a p value of 0.8. The correlation between MSS and RMT ratio within the four ethnical groups showed a p value between 0.3 and 0.8 and a rho between 0.8 and − 0.8. The Kruskal–Wallis test showed an eta2 of 0.13 and a p value of 0.0002 for the effect of ethnicity on MSS and an eta2 of 0.009 and a p value of 0.68 for the effect of ethnicity on the RMT ratio.
Conclusion: CS laparotomy scars heal differently between ethnical groups, but generally with satisfying results. Ethnicity does not affect myometrial healing and scar appearance does not reflect myometrial healing after CS. Thus, separate uterine sonographic assessment is recommended.
Aim: The aim of this study is to utilize the niche measurement guidelines outlined by Jordans et al. in order to establish normal values and accurate description of caesarean section scars in a normal population. After defining the normal distribution, abnormal pregestational scar characteristics will be identified for predicting adverse pregnancy outcomes. Methods: This is a prospective observational multicenter clinical study where women with a history of only one caesarean section and yet open family planning are enrolled. The uterine length, cervical length, niche length, niche depth, niche width, residual myometrial thickness, endometrial thickness, scar to internal os distance, anterior myometrial thickness superior and inferior to the scar and the posterior myometrial thickness opposite the scar, superior and inferior to it are measured in a pregestational uterus. The lower uterine segment is measured over a length of 3 cm during subsequent pregnancy and followed up until delivery. Results: Data from 500 patients will yield normal distribution curves for all predefined measurements. Establishing a correlation between deviations from the normal measures and adverse events would be instrumental for counseling women regarding subsequent pregnancy and mode of delivery.
Conclusion: This study will demonstrate the changes of the post-caesarean scar from a non-pregnant uterus until delivery and can confirm the importance of the scar characteristics in predicting pregnancy outcome.
Purpose: We aim to describe the sonographic uterine anatomy after a cesarean section (CS), test the reproducibility of predefined measurements from the BSUM study, and report the distribution of these measurements. Methods: This is a descriptive observational study where 200 women with a history of only one CS were recruited 12–24 months postoperatively. A 5–13 MHz micro-convex transvaginal transducer was used for the acquisition of volumetric datasets for evaluating the CS scars. We defined 15 distinct measurements including the residual myometrial thickness (RMT). RMT ratio was calculated as a percentage of RMT to the assumed pre-cesarean anterior uterine wall thickness. A P value below 0.05 is utilized for significant statistical analysis. Results: Patients were included on average 18.5 months post-cesarean. The uterus was anteflexed in 82.5% and retroflexed in 17.5%. Myometrial defects at the site of CS manifest in two forms, either as a niche or as fibrosis. Patients are classified into four groups: those with isolated niches (45%), combined niches and fibrosis (38.5%), isolated fibrosis (11%), and lacking both (5%). The median RMT ratio for these groups was 63.09, 40.93, 59.84, and 100% with a standard deviation of 16.73, 12.95, 16.59, and 0, respectively. The interclass correlation coefficient (ICC) remained above 0.9 for all distinct measurements among these groups except for those of RMT, where ICC varied between 0.47 and 0.96. The RMT ratio shows a constant ICC at 0.94 regardless of the group. Conclusion: The post-cesarean uterus is often anteflexed, and a myometrial loss of about 50% is normally expected. The pattern of this loss is in the form of a predominantly sharp-edged and echogenic niche, fibrosis, or a combination of both. The proposed RMT ratio takes these changes into consideration and results in a reproducible quantification. We hypothesize that different adverse outcomes could be attributed to the different scar patterns.
Die schlechte Prognose des Plattenepithelkarzinoms der Kopf-Hals-Region zeigt die Notwendigkeit, neue Therapieverfahren zu erforschen und die vorhandenen Behandlungsmodalitäten zu optimieren und miteinander bzw. mit neuen Therapieverfahren zu kombinieren. Die intraarterielle Chemotherapie als Chemoperfusion bildet im Rahmen der Optimierung der vorhandenen ein Therapieverfahren, das eine hohe lokale Effektivität bei Kopf-Halstumoren aufweist. Die Chemoembolisation als eine weitere Optimierung der intraarteriellen Chemotherapie ist eine innovative und vielversprechende Methode. Hauptgegenstand dieser Untersuchung war es, mit Hilfe der Mikrodialyse als einem wenig invasiven Instrument zur dynamischen Messung der extrazellulären Gewebekonzentrations-Zeitverläufe die Konzentration von Cisplatin und seinem Antidot Natriumthiosulfat bei unterschiedlichen Applikationswegen (Cisplatin intraarteriell und Natriumthiosulfat intravenös) in Abhängigkeit von der Applikationsverfahren, d.h. Chemoperfusion versus Chemoembolisation, zu bestimmen. Die im Blutkreislauf gemessenen Konzentrationen wurden dem gegenübergestellt. Ziel war die Optimierung der systemischen Abschirmung des lokal wirksamen Cisplatin bei der höchstmöglichen lokalen Cisplatin-Konzentration. Es wurden zwei Patientengruppen untersucht. Die Patientenaufnahme in die Studie erfolgte konsekutiv. Die erste Gruppe bestand aus 10 Patienten, die zweite Gruppe aus 6 Patienten. Die histologisch gesicherten Plattenepithelkarzinome waren, mit einer Ausnahme, nicht vorbehandelt. Eine Cisplatindosis von 150 mg/m² wurde für die Behandlung beider Gruppen eingesetzt. Die Applikation der neoadjuvanten Chemotherapie erfolgte superselektiv über einen transfemoralen Zugang nach angiographischer Darstellung. In der ersten Gruppe wurde eine intraarterielle Cisplatin-Chemoperfusion eingesetzt, wobei die absolute Cisplatindosis in 500ml NaCl-Lösung 0,9% aufgelöst wurde. In der zweiten wurde eine Chemoembolisationsmethode eingesetzt unter Verwendung einer wässrigen kristallinen Cisplatinsuspension, wobei die Cisplatindosis in 45-60 ml NaCl-Lösung 0,9% aufgelöst wurde. 10 s nach Beginn der Cisplatingabe wurde Natriumthiosulfat mit einer Dosis von 9 g/m² intravenös verabreicht. Nach präinterventioneller Insertion einer Mikrodialysesonde in das Tumorzentrum wurden Dialysate in 20 minütigen Intervallen gesammelt. Parallel und über einen Zeitraum wurden bei der ersten Gruppe von bis 70 Stunden und bei der zweiten Gruppe von bis zu 8 Stunden post interventionem Blutproben entnommen. Die Konzentrationen von Cisplatin und Natriumthiosulfat im Dialysat und in den Blutproben wurden unter Verwendung von HPLC analysiert. Nach Chemoembolisation war die maximale Tumorkonzentration (Cmax=180,3 ± 62,3 μmol/L) und die Fläche unter der Konzentration/Zeit-Kurve (AUC= 89,6 ± 31,3 μM·h) für Cisplatin etwa 5 mal höher als die entsprechenden Werte für die intraarterielle Chemoperfusion (Cmax=37,6 ± 8,9 μmol/L, AUC=19,8 ± 4,7 μM·h). Die maximale Cisplatinkonzentration im Plasma war Cmax=4,8 ± 0,6 μmol/L nach der Chemoperfusion und Cmax=0,9 ± 0,2 μmol/L nach der Chemoembolisation. Im Gegensatz zu den Verhältnissen zwischen Cisplatintumor- und Cisplatinplasmakonzentrationen waren die maximale Natriumthiosulfat-Konzentrationen und die AUC für Natriumthiosulfat unabhängig von der Verabreichungsmodalität signifikant höher im Plasma (Cmax=5051 ± 381 μmol/L; AUC= 5589 ± 269 μM·h) als im Tumorgewebe (Cmax=1686 ± 151 μmol/L; AUC= 2204 ± 167 μM·h). Nach der Chemoperfusion waren die durchschnittlichen Quotienten der AUC von Natriumthiosulfat zur AUC von Cisplatin im Tumor und Plasma 211 ± 75 und 984 ± 139, nach der Chemoembolisation waren die entsprechenden Werte 48,5 ± 29,5 und 42.966 ± 26.728. Die extratumoralen molaren Quotienten von Natriumthiosulfat zu Cisplatin müssen > 500 betragen, um Cisplatin zu neutralisieren. Dies wurde in beiden Darreichungsformen erreicht. Intratumoral sollte dieses Verhältnis 100 nicht übersteigen, um die tumorschädigenden Cisplatineigenschaften nicht aufzuheben. Dieses Ziel wurde nur in der Chemoembolisationsgruppe erreicht. Der pharmakokinetische Vorteil der intraarteriellen Cisplatinchemoembolisation gegenüber der intraarteriellen Chemoperfusion hat sich in der vorliegenden Studie bestätigt. Ebenso fanden sich hohe lokale Cisplatinkonzentrationen nach intraarterieller Applikation im Vergleich zu aus der Literatur bekannten Werten nach intravenöser Applikation. Die klinische Wirksamkeit im Bezug auf die Tumorantwort und die Überlebensraten soll in größeren Patientenkollektiven untersucht werden. Ferner soll die Chemoembolisation so optimiert werden, dass weniger modalitätsbedingten Nebenwirkungen auftreten.
Background: Leishmaniasis is a chronic disease that is found in various countries of the world. The aim of the current study was to investigate the impact of leishmaniasis on the world's research output. The present study assessed benchmarking of research output for the period between 1957 and 2006. Using large database analyses, research in the field of leishmaniasis was evaluated. Furthermore, cooperation between different countries was identified.
Results: The number of publications increased with time. Most publications came from Western countries such as the US, UK or Germany. Interestingly, countries like Brazil and India had a high research output. We found a substantial amount of cooperation between countries.
Conclusion: Although leishmaniasis is of limited geographic distribution it attracts a wide research interest. The central hub of research cooperation is the USA.
Background: Intracerebral haemorrhage growth is associated with poor clinical outcome and is a therapeutic target for improving outcome. We aimed to determine the absolute risk and predictors of intracerebral haemorrhage growth, develop and validate prediction models, and evaluate the added value of CT angiography.
Methods: In a systematic review of OVID MEDLINE—with additional hand-searching of relevant studies' bibliographies— from Jan 1, 1970, to Dec 31, 2015, we identified observational cohorts and randomised trials with repeat scanning protocols that included at least ten patients with acute intracerebral haemorrhage. We sought individual patient-level data from corresponding authors for patients aged 18 years or older with data available from brain imaging initially done 0·5–24 h and repeated fewer than 6 days after symptom onset, who had baseline intracerebral haemorrhage volume of less than 150 mL, and did not undergo acute treatment that might reduce intracerebral haemorrhage volume. We estimated the absolute risk and predictors of the primary outcome of intracerebral haemorrhage growth (defined as >6 mL increase in intracerebral haemorrhage volume on repeat imaging) using multivariable logistic regression models in development and validation cohorts in four subgroups of patients, using a hierarchical approach: patients not taking anticoagulant therapy at intracerebral haemorrhage onset (who constituted the largest subgroup), patients taking anticoagulant therapy at intracerebral haemorrhage onset, patients from cohorts that included at least some patients taking anticoagulant therapy at intracerebral haemorrhage onset, and patients for whom both information about anticoagulant therapy at intracerebral haemorrhage onset and spot sign on acute CT angiography were known.
Findings: Of 4191 studies identified, 77 were eligible for inclusion. Overall, 36 (47%) cohorts provided data on 5435 eligible patients. 5076 of these patients were not taking anticoagulant therapy at symptom onset (median age 67 years, IQR 56–76), of whom 1009 (20%) had intracerebral haemorrhage growth. Multivariable models of patients with data on antiplatelet therapy use, data on anticoagulant therapy use, and assessment of CT angiography spot sign at symptom onset showed that time from symptom onset to baseline imaging (odds ratio 0·50, 95% CI 0·36–0·70; p<0·0001), intracerebral haemorrhage volume on baseline imaging (7·18, 4·46–11·60; p<0·0001), antiplatelet use (1·68, 1·06–2·66; p=0·026), and anticoagulant use (3·48, 1·96–6·16; p<0·0001) were independent predictors of intracerebral haemorrhage growth (C-index 0·78, 95% CI 0·75–0·82). Addition of CT angiography spot sign (odds ratio 4·46, 95% CI 2·95–6·75; p<0·0001) to the model increased the C-index by 0·05 (95% CI 0·03–0·07).
Interpretation: In this large patient-level meta-analysis, models using four or five predictors had acceptable to good discrimination. These models could inform the location and frequency of observations on patients in clinical practice, explain treatment effects in prior randomised trials, and guide the design of future trials.

Background: One of the lesser recognized complications of diabetes mellitus are musculoskeletal (MSK) complications of the upper and lower extremity. No prevalence studies have been conducted in general practice. Thus, the aim of this study was to investigate the prevalence of upper extremity MSK disorders in patients with type 2 diabetes (T2DM) in the Netherlands. Methods: We conducted a cross-sectional study with two different approaches, namely a representative Dutch primary care medical database study and a questionnaire study among patients with T2DM. Results: In the database study, 2669 patients with T2DM and 2669 non-diabetes patients were included. MSK disorders were observed in 16.3% of patients with T2DM compared to 11.2% of non-diabetes patients (p < 0.001, OR 1.53, 95% CI 1.31, 1.80). In the questionnaire study, 200 patients with T2DM were included who reported a lifetime prevalence of painful upper extremity body sites for at least four weeks of 67.3%. Conclusion: We found that upper extremity MSK disorders have a high prevalence in Dutch patients with T2DM presenting in general practice. The prevalence ranges from 16% based on GP registered disorders and complaints to 67% based on self-reported diagnosis and pain. Early detection and treatment of these disorders may play a role in preventing the development of chronic MSK disorders.
It has been recognized that molecular classifications will form the basis for neuropathological diagnostic work in the future. Consequently, in order to reach a diagnosis of Alzheimer's disease (AD), the presence of hyperphosphorylated tau (HP-tau) and beta-amyloid protein in brain tissue must be unequivocal. In addition, the stepwise progression of pathology needs to be assessed. This paper deals exclusively with the regional assessment of AD-related HP-tau pathology. The objective was to provide straightforward instructions to aid in the assessment of AD-related immunohistochemically (IHC) detected HP-tau pathology and to test the concordance of assessments made by 25 independent evaluators. The assessment of progression in 7-µm-thick sections was based on assessment of IHC labeled HP-tau immunoreactive neuropil threads (NTs). Our results indicate that good agreement can be reached when the lesions are substantial, i.e., the lesions have reached isocortical structures (stage V–VI absolute agreement 91%), whereas when only mild subtle lesions were present the agreement was poorer (I–II absolute agreement 50%). Thus, in a research setting when the extent of lesions is mild, it is strongly recommended that the assessment of lesions should be carried out by at least two independent observers.
Hintergrund und Fragestellung Die dermatologische Ausbildung von Studierenden an der Johann Wolfgang Goethe-Universität in Frankfurt am Main wird seit dem Wintersemester 2002/2003 durch ein teilweise verpflichtendes Onlineangebot ergänzt. Wichtigster Bestandteil der Onlinelehre ist die verpflichtende Bearbeitung von virtuellen Patientenfällen. Durch die Bearbeitung soll die Vorgehensweise der Patientenbehandlung (Anamnese erheben → dermatologischen Befund beschreiben→ Diagnose erstellen → Therapieplan erarbeiten) simuliert und trainiert werden. Ziel der Untersuchungen Die vorliegende Promotion geht der Frage nach, wie das in die Präsenzlehre integrierte Onlineangebot genutzt wurde und wie die Qualität der Bearbeitung zu bewerten ist. Zusätzlich wurde die Akzeptanz der Onlinelehre in Form von Evaluationen hinterfragt. Die ausgewerteten elektronischen Benutzerprotokolle (Log-Files) und Evaluationsergebnisse dienten der Überarbeitung des Onlineangebots zum Wintersemester 2006/2007. Zusätzlich wurden die Ergebnisse der für das Wintersemester 2006/2007 geänderten Inhalte untersucht und bewertet. Material und Methode Die durch die Nutzung des Onlineangebots automatisch aufgezeichneten elektronischen Benutzerprotokolle (Log-Files) wurden retrospektiv ausgewertet. Dabei wurden die Nutzungsdaten von 805 Studierenden, die das Onlineangebot nur mittels eines individuellen Passworts nutzen konnten, untersucht. Es erfolgte keine personalisierte Auswertung. Die Datenerhebung erfolgte ab dem Wintersemester 2002/2003. Ausgewertet wurden die Nutzungsdaten folgender Wintersemester: 2003/2004, 2004/2005, 2005/2006, 2006/2007. Weiterhin wurden die Evaluationsergebnisse der Onlineevaluationen, die im Anschluss an folgende Wintersemester erfolgte: 2003/2004, 2004/2005, 2005/2006 analysiert. Zudem fand eine persönliche Befragung von 10 Studierenden bezüglich des Onlineangebots im Sommersemester 2006 statt. Ergebnisse * Anzahl der Fallbearbeitungen Durchschnittliche bearbeitete jeder Kursteilnehmer 4,4 virtuelle Patientenfälle. Somit wurden 10 % aller Partienfälle freiwillig bearbeitet. Dabei fand die Fallbearbeitung vorzugsweise einen Tag vor der Freischaltung des nächsten Patientenfalls statt. * Durchschnittliche Bearbeitungszeit Die Auswertungen ergaben, dass die Bearbeitungszeit eines virtuellen Patientenfalls von der Anzahl der Einsendeabfragen und der Fallpositionierung bei der Präsentation abhängig ist. Dabei beträgt die Fragenbearbeitungszeit des jeweils erstpositionierten Patientenfalls ca. 6 Minuten und vermindert sich auf ca. 3 Minuten beim letztpositionierten Patientenfall. * Die inhaltliche Auswertung der Onlinefälle Die inhaltliche Auswertung der virtuellen Patientenfälle und die Ergebnisse der Fallevaluationen zeigten einen Optimierungsbedarf hinsichtlich der Fragestellung einzelner Fragen. Daher wurden 13 der insgesamt 49 Fallfragen zum Wintersemester 2006/2007 überarbeitet (26,5 %). Die Überarbeitung führte im Wintersemester 2006/2007 bei 9 Fragen zu signifikant höheren Ergebnissen. Bei 4 Fragen, die „Transferleistungen“ erforderten, kam es zu keiner signifikanten Verbesserung der Ergebnisse. Die Freitext-Fragen der virtuellen Patientenfälle wurden auch ohne konkrete elektronische Korrektur ernst genommen und so gut wie möglich von 99,4 % der Studierenden beantwortet. Dieses Ergebnis zeigte, dass die Selbstkorrektur im Anschluss an die Fragenbearbeitung ausreichend war. Zudem wurde deutlich, dass der Präsentationszeitpunkt eines virtuellen Patientenfalls keinen Einfluss auf die durchschnittlichen Gesamtergebnisse hatte. * Befundbeschreibungsergebnisse Bezüglich der Befundbeschreibungen zeigten sich grundsätzlich drei Probleme: 1. Die Befundbeschreibungen waren nicht vollständig 2. Es zeigten sich Defizite bei der Benennung der richtigen Effloreszenz 3. Die Bedeutung des Übungsbereichs „Befundbeschreibungen“ wurde nicht erkannt * Evaluationsergebnisse des Onlineangebots Die Evaluationsergebnisse ergaben eine positive Bewertung des Onlineangebots durch die Studierenden, obwohl das Onlineangebot teilweise verpflichtend war. Dabei gaben ca. 90 % der Studierenden an, durch die Bearbeitung der Onlinefälle, einen Lerneffekt erzielt zu haben. Auch das Verhältnis zwischen Präsenzveranstaltung und Onlineunterricht wurde von ca. 90% der Studierenden positiv bewertet. Zudem bezeichneten ca. 80 % der Studierenden die Verzahnung der Onlinelehre („Hausaufgaben“) mit der Präsenzlehre („interaktive Vorlesung“) als motivationsfördernd. Schlussfolgerungen Integrierte verpflichtende Online-Angebote mit sinnvoll auf die Präsenzlehre abgestimmten Inhalten (Blended-Learning) werden gut bewertet. Ebenfalls gut bewertete, singuläre freiwillige Online-Angebote werden dagegen nur vereinzelt genutzt. Wichtige Inhalte sollten "Online" zu Kursbeginn angeboten werden, da die Bearbeitungszeit im Laufe des Semesters sinkt. Die Positionierung der Online-Elemente vor die Präsenzlehre erhöht die Bearbeitungszeit und verbessert die Gesamtergebnisse. Durch Umformulierung ließen sich die Ergebnisse bei 9 Fragen signifikant verbessern. Das vorhandene Lehrangebot konnte die Fähigkeiten der Studierenden zur Befundbeschreibung nicht verbessern. Evaluationen wie diese können damit Schwachstellen vorhandener Lehrangebote identifizieren und dabei helfen, diese zu optimieren.
Background and aims: Despite the clinical importance of atherosclerosis, the origin of cells within atherosclerotic plaques is not fully understood. Due to the lack of a definitive lineage-tracing strategy, previous studies have provided controversial results about the origin of cells expressing smooth muscle and macrophage markers in atherosclerosis. We here aim to identify the origin of vascular smooth muscle (SM) cells and macrophages within atherosclerosis lesions.
Methods: We combined a genetic fate mapping approach with single cell expression analysis in a murine model of atherosclerosis.
Results: We found that 16% of CD68-positive plaque macrophage-like cells were derived from mature SM cells and not from myeloid sources, whereas 31% of αSMA-positive smooth muscle-like cells in plaques were not SM-derived. Further analysis at the single cell level showed that SM-derived CD68+ cells expressed higher levels of inflammatory markers such as cyclooxygenase 2 (Ptgs2, p = 0.02), and vascular cell adhesion molecule (Vcam1, p = 0.05), as well as increased mRNA levels of genes related to matrix synthesis such as Col1a2 (p = 0.01) and Fn1 (p = 0.04), than non SM-derived CD68+ cells.
Conclusions: These results demonstrate that smooth muscle cells within atherosclerotic lesions can switch to a macrophage-like phenotype characterized by higher expression of inflammatory and synthetic markers genes that may further contribute to plaque progression.
Stroke is a major public health issue worldwide. The prevalence of stroke in 2010 was 33 million, with 16.9 million people having a first stroke.1 Stroke was the second‐leading cause of death behind heart disease globally, accounting for over 10% of total deaths worldwide.
Stroke is a heterogeneous condition that can be due to rupture of a blood vessel (hemorrhagic) or to blockage of a vessel (ischemic). About 85% of strokes are ischemic in origin and these are often classified by mechanism. This should be distinguished from risk factors such as hypertension, diabetes, smoking, etc. Risk factors increase the risk of stroke but do not necessarily explain the mechanism of a particular stroke. About 25% of ischemic strokes have a radiographic appearance similar to that seen in patients with cardioembolic sources (such as atrial fibrillation [AF], prosthetic valves, valvular prolapse, or mitral valve regurgitation), but no embolic source is found. These "cryptogenic strokes" (CS; also called embolic strokes of undetermined source) pose a particular clinical challenge in that the optimal antithrombotic therapy to reduce recurrence is uncertain. Since there are currently no data to support long‐term oral anticoagulation (OAC) in CS, but also no specific trials that have addressed this question, guidelines recommend antiplatelet therapy. Identification of AF in these patients changes the most likely mechanism to cardioembolism, and thus changes the recommended antithrombotic therapy to OAC, which is extremely effective in preventing stroke in patients with AF.
This report is based on discussions held at The Diagnostics and Monitoring Stroke Focus Group, a meeting held on January 15 to 17, 2015. The meeting focused on CS as a healthcare issue, and the utility of extended cardiac monitoring for AF in patients with strokes of unknown origin. The objectives of the meeting were to review existing information on the subject, define areas where knowledge was lacking or limited, and discuss study designs by which information gaps might be filled.
Small bowel tumors are detected in approximately 10% of patients with small bowel endoscopies for obscure or overt mid-intestinal bleeding. Small bowel tumors may be of malignant or benign etiology. Malignant etiologies include adenocarcinoma, neuroendocrine tumors, or lymphoma, whereas benign lesions are typically lipomas, inflammatory polyps, or adenomas. Within the group of nonneoplastic lesions inflammatory polyps are most frequent. Significant bleeding and bowel obstruction due to intussusception might occur, and surgical or endoscopic treatment has been reported for symptomatic patients. The small bowel is essential to sustain alimentation and small bowel Crohn's disease (CD) may severely limit its function. Small bowel imaging is a crucial element in diagnosing small
bowel CD, and treatment control with imaging is increasingly used to optimize the patients outcome. Thereby, capsule endoscopy, Balloon-assisted enteroscopy, and Magnetic resonance imaging have become key players to manage CD patients. In this review, role of small bowel imaging is detailed discussed for use in diagnosing and managing Crohn's disease patients.
Endoscopic retrograde cholangiopancreatography (ERCP) offers an effective interventional option for treating symptomatic chronic pancreatitis. Endoscopic pancreatic sphincterotomy is performed to facilitated endoscopic treatment. Pancreatic duct strictures can be treated by inserting plastic stents, and a 10 Fr endoprosthesis is adequate in many cases. Before stent insertion, hydrostatic balloon dilation is needed in some cases. Pancreatic stones can be removed with a dormia basket, but combining ERCP and extracorporeal shockwave lithotripsy (ESWL) is often most effective.
Standard and advanced endoscopic treatment approaches are delineated in this article and include stricture dilation with a Soehendra retriever, cSEMS placement and multi-stenting.
Celiac disease (CD) is an immune-mediated enteropathy that is characterized by intraepithelial lymphocytosis, crypt hyperplasia, and villous atrophy. Prevalence is high and has been estimated to range between 0.5% and 1.5%. Capsule endoscopy (CE) has a sensitivity and specificity of approximately 90%. CD is an important differential diagnosis for diagnostic workup for anemia, malabsorption, or diarrhea, and must be recognized reliably by the investigator. Moreover, CE is the preferred method to screen for complications in CD, such as enteropathy-associated T-cell lymphoma, ulcerative jejunitis, and small bowel adenocarcinoma. Peutz–Jeghers syndrome (PJS) is a rare autosomal-dominant inherited disorder characterized by gastrointestinal hamartomas, mucocutaneous pigmentation, and an elevated cancer risk. Moreover, intussusception risk may be as high as 50% at the age of 20 years and is caused by large polyps. There is some evidence that endoscopic surveillance of PJS patients with removal of small intestinal polyps with a diameter of more than 15 mm efficiently prevents intussusceptions. In recent years, capsule endoscopy (CE) has largely replaced small-bowel radiography techniques to screen for small-bowel polyps. Magnetic resonance imaging may be equally efficient as CE for screening of large polyps. Balloon enteroscopy may be used for endoscopic snare resection of polyps. The author presents the case of a patient with severe bleeding from a duodenal ulcer that could not be controlled by endoscopic application of metal clips and injection of fibrin glue. Angiographic embolization with placement of coils into the feeding vessel stopped the bleeding. However, 3 days later, a fistula emerged from coil material penetrating into the dorsal duodenum and a peritoneal leakage developed. Small-bowel tumors are rare and account for approximately 5% of all gastrointestinal tumors. Approximately 65% of small-bowel tumors are malignant, and approximately 40% of these tumors are adenocarcinomas. Similar to colorectal adenocarcinoma, premalignant adenomas of the small bowel may progress to carcinoma. This occurs both sporadically and in the context of hereditary tumor syndromes such as familial adenomatous polyposis or hereditary nonpolyposis colorectal cancer (Lynch syndrome). Small bowel varices may be found in less than 5% of patients with suspected small bowel bleeding. These varices are associated with portal hypertension or thrombosis of mesenteric venous vessels and with altered abdominal vascular anatomy with or without prior small bowel surgery. In bleeding small bowel varices, therapeutic options include endoscopic injection of tissue adhesives, endovascular approaches such as balloon-occluded retrograde transvenous or percutaneous obliteration and transjugular intrahepatic portosystemic shunt, and surgical resection. This is a case report of a 53-year-old patient with ethylic liver cirrhosis who presented with severe, life-threatening hematochezia due to small bowel varices. Here the authors report the case of an elderly woman who had upper abdominal pain, upper gastrointestinal hemorrhage, and jaundice (a symptomatic triad termed the ‘Quincke’ triad) a few days after endoscopic sphincterotomy. Abdominal ultrasonography demonstrated an echo-rich filling of the choledochus consistent with hemobilia. Endoscopic retrograde cholangiography was immediately performed and blood clots were removed from the common bile duct. A nasobiliary catheter was placed to irrigate the bile duct for prevention of recurring obstruction of the bile ducts from blood clots. Further follow-up of the patient was uneventful. Operatively altered anatomy might provide a challenge for endoscopic retrograde cholangiopancreatography. However, with the support of the balloon-assisted enteroscopy technique the access route to the biliary system even in long-limb Roux-Y anastomosis is feasible in most cases.
In this video case report, an 81-year-old woman was symptomatic for stone obstruction of the common bile duct (CBD). Complete gastrectomy had been performed in this patient for stomach cancer many years earlier. Balloon-assisted enteroscopy was used for retrograde access of the duodenum via a Roux-Y anastomosis. There was major difficulty in intubating the CBD via the native papilla in this case because access was prevented by the tangential approach of the enteroscope. Stent insertion is an established technique of endoscopic retrograde cholangiopancreatography (ERCP) to treat symptomatic malignant or benign biliary strictures, and stent placement is accomplished by using the over-the-wire (OTW) method. In some cases, however, it might be challenging and sometimes time consuming to pass a complex biliary stricture with the guidewire. Stent-exchange technique with a guidewire left in place during stent removal might therefore be helpful to guarantee successful and time-sparing interventions.

Im Zeitraum von 1991 - 1992 wurden 2000 Stuhlproben von a) Tropenreisenden (n = 600), b) Patienten mit Durch fall (n = 500) und c) Patienten mit gastrointestinalen Symptomen, jedoch ohne Durchfall (n = 900) auf Campylobacter spp. untersucht Hierfür wurden drei Kultivierungsmethoden parallel eingesetzt: 7. eine Filtermembranmethode, 2. ein blutfreies Campylobacter-Selektivmedium und 3. ein Blutmedium mit Butzler-Supplement Zusätzlich wurde der Einfluß von Transportbedingungen untersucht mittels Vergleiches zwischen Nativ-Stuhlproben versus Stühlen, die in ein Transportnährmedium (Cary-Blair-Medium) gegeben worden waren (n = 517).
Insgesamt waren 54/2000 (2,7%) Stuhlproben positiv. Die Campylobacter-Auffindungsrate in flüssigen Stühlen (Gruppe B) lag dreifach höher im Vergleich zu nicht-durchfälligen Proben (p < 0.01). In den Gruppen b) und c) waren Patienten, die älter als 40 Jahre waren, signifikant seltener mit Campylobacter infiziert als jüngere Personen, während es bei den Tropenreisenden keinen Alterseffekt gab. Unter den drei Kultivierungsmethoden war das Filtermembranverfahren dasjenige mit der besten Campylobacter-Ausbeute (p < 0.01): Methode 1 ergab 57,5%, Methode 2 50% und Methode 3 40,7% positive Isolate. Durch den Einsatz von Cary-Blair-Transportmedium konnte die Campylobacter-Isolierungsrate um 25% gesteigert werden. Die am häufigsten isolierten Spezies waren C. jejuni und C. coli.
In dieser prospektiven, randomisierten, doppelblinden und plazebo-kontrollierten Studie wurde an einem Kollektiv von 23 Hausstaubmilben-Allergikern (Grad 1° - max. 2° nach WHO/GINA) die entzündungshemmende Wirkung einer 5-wöchigen n-3 PUFA Supplementierung (0,69 g/ Tag) nach niedrig dosierten Allergeninhalationen untersucht. Die Probanden wurden durch Randomisierung entweder auf die Gruppe A (n=12) oder auf die Gruppe B (n=11) verteilt. Gruppe A nahm über einen Zeitraum von 5 Wochen eine Fettsäuremischung aus EPA 450 mg/Tag, DHA 180 mg/Tag, GLA 60 mg/Tag und STA 60 mg/Tag ein. Gruppe B wurde über den gleichen Zeitraum mit einer Kontroll-Fettsäuremischung aus ungesättigten und einfach gesättigten Fettsäuren supplementiert. Nach 3 Wochen wurden die Probanden täglich an 5 aufeinander folgenden Tagen in 2 zusammenhängenden Wochen mit niedrig dosierten Milbenallergenen provoziert. Die Sputuminduktion erfolgte bei den Visiten 1 (V1), 2 (V2) und 12 (V12) jeweils eine Stunde nach den Provokationen, gemäß einer standardisierten, bereits vorbeschriebenen Technik. Blutproben wurden vor der Supplementierung, nach 3 Wochen und nach ein- und zweiwöchiger Allergenprovokation entnommen. Das Sputum wurde nach der Induktion sofort aufbereitet und die ECP-Konzentrationen im Sputumüberstand und in den Serumproben mit Hilfe eines Chemiluminescence-Immunoassay (Biermann, Bad Nauheim, Deutschland) bestimmt. Es konnte gezeigt werden, dass die EPA-Konzentrationen im Plasma und in den Erythrozytenmembranen während der Supplementierung signifikant höhere Werte in der n-3 PUFA-Gruppe aufwiesen, was für eine gute Compliance der Probanden über den gesamten Supplementierungszeitraum spricht. Außerdem war der Prozentsatz an eosinophilen Granulozyten im Sputum nach täglicher Allergenprovokation in der Plazebogruppe tendenziell größer als in der n-3 PUFA Gruppe, die Anstiege der ΔECP-Konzentrationen im Sputum beider Gruppen verfehlten nur knapp die Signifikanzgrenze (p < 0,06 und p < 0,09). Der prozentuale Anteil der Eosinophilen im Serum unter n-3 PUFA Supplementierung war deutlich geringer verglichen mit der Plazebogruppe. Innerhalb der Plazebogruppe kam es bei dem prozentualen Anteil eosinophiler Granulozyten im Serum zu einem signifikanten Anstieg zwischen Visite 2 und Visite 7 (p = 0,002), bzw. zwischen Visite 2 und Visite 12 (p = 0,01). Ebenso kam es innerhalb der n-3 PUFA Gruppe bei dem prozentualen Anteil eosinophiler Granulozyten im Serum zu einem signifikanten Anstieg zwischen Visite 2 und Visite 7 (p = 0,003), bzw. zwischen Visite 2 und Visite 12 (p = 0,02). Beim Vergleich zwischen der Plazebo- und der n-3 PUFA Gruppe fällt ein signifikanter Unterschied bezüglich des prozentualen Anteils der eosinophilen Granulozyten im Serum bei Visite 7 (p = 0,02) und bei Visite 12 (p = 0,04) mit niedrigeren Werten in der n-3 PUFA-Gruppe auf. Verglichen mit der Plazebogruppe waren die ΔECP-Serum-Konzentrationen in der n-3 PUFA Gruppe nach einer Woche Allergeninhalation bei Visite 7 signifikant niedriger (Plazebo: 20,52 ng/ml ± 9,93; n-3 PUFA: -1,68 ng/ml ± 4,36; p < 0,05). Dieser Unterschied war nach 2-wöchiger Allergeninhalation bei Visite 12 immer noch deutlich vorhanden, erreichte aber nicht mehr das Signifikanzniveau (Plazebo: 158,46 ng/ml ± 58,17, n-3 PUFA: 131,63 ng/ml ± 33,66; p >0,05). In beiden Gruppen war jedoch im Verlauf zwischen Visite 7 und Visite 12 ein signifikanter Anstieg der ΔECP-Serum-Konzentrationen im Serum zu verzeichnen. Zusammenfassend kann man sagen, dass die Supplementierung mit n-3 PUFA aufgrund dieser Ergebnisse als eine mögliche Methode zur entzündungshemmenden, nicht medikamentösen Therapie bei Patienten mit leichtgradigem Asthma bronchiale anzusehen ist. Unsere Untersuchung liefert Hinweise, dass eine Reduzierung der bronchialen Entzündungsreaktion mittels einer 5-wöchigen täglichen Nahrungsergänzung mit 0,69 g n-3 PUFA bei Probanden mit allergischem Asthma möglich ist.

Chimeric antigen receptor (CAR)-T cell therapies are on the verge of becoming powerful immunotherapeutic tools for combating hematological diseases confronted with pressing medical needs. Lately, CAR-NK cell therapies have also come into focus as novel therapeutic options to address hurdles related to CAR-T cell therapies, such as therapy-induced side effects. Currently, more than 500 CAR-T and 17 CAR-NK cell trials are being conducted worldwide including the four CAR-T cell products Kymriah, Yescarta, Tecartus and Breyanzi, which are already available on the market. Most CAR-T cell-based gene therapy products that are under clinical evaluation consist of autologous enriched T cells, whereas CAR-NK cell-based approaches can be generated from allogeneic donors. Besides modification based on a second-generation CAR, more advanced CAR-immune cell therapeutics are being tested, which utilize precise insertion of genes to circumvent graft-versus-host disease (GvHD) or employ a dual targeting approach and adapter CARs in order to avoid therapy resistance caused by antigen loss. In this review, we are going to take a closer look at the commercial CAR-T cell therapies, as well as on CAR-T and CAR-NK cell products, which are currently under evaluation in clinical trials, that are being conducted in Germany.
Acute myeloid leukemia (AML) is a malignant disorder derived from neoplastic myeloid progenitor cells characterized by abnormal proliferation and differentiation. Although novel therapeutics have recently been introduced, AML remains a therapeutic challenge with insufficient cure rates. In the last years, immune-directed therapies such as chimeric antigen receptor (CAR)-T cells were introduced, which showed outstanding clinical activity against B-cell malignancies including acute lymphoblastic leukemia (ALL). However, the application of CAR-T cells appears to be challenging due to the enormous molecular heterogeneity of the disease and potential long-term suppression of hematopoiesis. Here we report on the generation of CD33-targeted CAR-modified natural killer (NK) cells by transduction of blood-derived primary NK cells using baboon envelope pseudotyped lentiviral vectors (BaEV-LVs). Transduced cells displayed stable CAR-expression, unimpeded proliferation, and increased cytotoxic activity against CD33-positive OCI-AML2 and primary AML cells in vitro. Furthermore, CD33-CAR-NK cells strongly reduced leukemic burden and prevented bone marrow engraftment of leukemic cells in OCI-AML2 xenograft mouse models without observable side effects.
BACKGROUND: The analysis of microarray time series promises a deeper insight into the dynamics of the cellular response following stimulation. A common observation in this type of data is that some genes respond with quick, transient dynamics, while other genes change their expression slowly over time. The existing methods for detecting significant expression dynamics often fail when the expression dynamics show a large heterogeneity. Moreover, these methods often cannot cope with irregular and sparse measurements.
RESULTS: The method proposed here is specifically designed for the analysis of perturbation responses. It combines different scores to capture fast and transient dynamics as well as slow expression changes, and performs well in the presence of low replicate numbers and irregular sampling times. The results are given in the form of tables including links to figures showing the expression dynamics of the respective transcript. These allow to quickly recognise the relevance of detection, to identify possible false positives and to discriminate early and late changes in gene expression. An extension of the method allows the analysis of the expression dynamics of functional groups of genes, providing a quick overview of the cellular response. The performance of this package was tested on microarray data derived from lung cancer cells stimulated with epidermal growth factor (EGF).
CONCLUSION: Here we describe a new, efficient method for the analysis of sparse and heterogeneous time course data with high detection sensitivity and transparency. It is implemented as R package TTCA (transcript time course analysis) and can be installed from the Comprehensive R Archive Network, CRAN. The source code is provided with the Additional file 1.
Sowohl die Lebenserwartung als auch die Prävalenz HIV-infizierter Patient*innen ist stetig ansteigend,aufgrund der HAART und durch verbesserte diagnostische Methoden. Nicht-AIDS-definierenden Erkrankungen sind heutzutage die führenden Todesursachen. Durch verzögerte Diagnosestellung und zurückhaltenden Therapien gynäkologischer Malignome ist die Prognose im Vergleich zur Normalbevölkerung schlechter.
In dieser retrospektiven Fall-Kontroll-Studie des Universitätsklinikums Frankfurt am Mains wurden die Therapie und das Outcome gynäkologischer Malignome von 23 HIV-infizierten Patientinnen aus den Jahren 2009-2019 mit einer Kontrollgruppe aus dem gynäkologischen Krebszentrum der Klinik Essen Mitte verglichen, um herauszufinden, inwiefern eine HIV-Infektion das Outcome der Patientinnen beeinflusst.
Das gynäkologische Malignom, dominierend das Zervixkarzinom, trat durchschnittlich ein Jahrzehnt nach der HIV-Diagnose auf. Im Unterschied zu anderen Studien, ist unser Kollektiv überwiegend hellhäutig. Ein bekannter Drogenabusus ist häufig und zusammenhängend mit weiteren Koinfektionen.
Die HIV-Erkrankung ist bei mehr als der Hälfte der Patientinnen bereits fortgeschritten, jedoch ließ sich kein Zusammenhang zwischen dem Auftreten gynäkologischer Malignome und einer CD4-Zellzahl <500 CD4-Zellen/µl nachweisen. Die antiretrovirale Therapie entsprach größtenteils nicht den aktuellen Leitlinien.
Bis auf fünf Frauen wurden alle Frauen leitliniengerecht therapiert. Eine Korrelation zwischen der Therapie und der Tumorentität, der CD4-Zellzahl, dem Alter oder dem Stadium des Malignoms konnte nicht gezeigt werden.
Insgesamt liegt die 5-Jahresüberlebensrate der Kohorte bei 74.8%. Eine nicht leitliniengerechte Therapie ist nicht direkt mit einem schlechteren Outcome verbunden, jedoch mit einem weitaus kürzeren Follow-Up-Zeitraum von durchschnittlich 0.22 Jahren im Vergleich zu 4.85 Jahren bei leitlinienkonform therapierten Patientinnen. Es liegt ein statistisch signifikanter Unterschied zwischen der Kontrollgruppe und unserer Kohorte vor, sodass angenommen werden kann, dass bei Vorliegen einer HIV-Infektion die Therapie des gynäkologischen Malignoms häufiger nicht leitliniengerecht ist.
Bislang existieren nur wenige Studien, die die Therapie und das Outcome gynäkologischer Malignome bei HIV-infizierten Patientinnen untersuchen. Die Interaktion einer ART mit antineoplastischen Medikamenten und die Anwendung von Checkpointinhibitoren und einer „targeted therapy“ sollten Gegenstand weiterer Untersuchungen sein. Dafür sollten HIV-Patientinnen in Therapiestudien inkludiert werden, sodass geeignete Leitlinien erarbeitet werden können.
Persistent neuropathic pain is a frequent consequence of peripheral nerve injuries, particularly in the elderly. Using the IntelliCage we studied if sciatic nerve injury obstructed learning and memory in young and aged mice, each in wild type and progranulin deficient mice, which develop premature signs of brain aging. Both young and aged mice developed long-term nerve injury-evoked hyperalgesia and allodynia. In both genotypes, aged mice with neuropathic pain showed high error rates in place avoidance acquisition tasks. However, once learnt, these aged mice with neuropathic pain showed a significantly stronger maintenance of the aversive memory. Nerve injury did not affect place preference behavior in neither genotype, neither in young nor aged mice. However, nerve injury in progranulin deficient mice impaired the learning of spatial sequences of awarded places, particularly in the aged mice. This task required a discrimination of clockwise and anti-clockwise sequences. The chaining failure occurred only in progranulin deficient mice after nerve injury, but not in sham operated or wildtype mice, suggesting that progranulin was particularly important for compensatory adaptations after nerve injury. In contrast, all aged mice with neuropathic pain, irrespective of the genotype, had a long maintenance of aversive memory suggesting a negative alliance and possibly mutual aggravation of chronic neuropathic pain and aversive memory at old age.
Gemeinsam ist den Autoimmunendokrinopathien, dass genetische Faktoren als auch Umweltfaktoren für die Ätiologie von Bedeutung sind. Das Vererbungsschema dieser genetischen Prädispositionsfaktoren ist komplex und schließt mehrere Kandidatengene ein. Besonders die HLA-Gene wurden eingehend in den letzten Jahren untersucht. Sie stellen einen wichtigen Risikofaktor für die Entstehung von Autoimmunendokrinopathien dar. Allerdings ist für keinen der bekannten Autoimmunendokrionopathien die genetische Prädisposition, die durch die HLA-Region vermittelt wird, weder ausreichend noch notwendig für die Entwicklung der Erkrankung. So nehmen weitere Kandidatengene eine bedeutende Rolle in der Entstehung von Autoimmunendokrinopathien ein. In den letzten Jahren haben Studien gezeigt, dass diejenigen Mechanismen des Immunsystems, die zur Erhaltung der peripheren T-Zell-Toleranz beitragen, essentiell für die Kontrolle von autoreaktiven T-Zellen sind. Die Elimination bzw. die Störung solcher Mechanismen kann somit in einer Autoimmunerkrankung münden. Folglich sind Gene, die Einfluss auf diese Mechanismen der peripheren Toleranz ausüben, eindeutige Prädispositionsfaktoren für die Entstehung von Autoimmunerkrankungen. CTLA-4 ist ein essentielles inhibitorisches Regulatormolekül für T-Zellen. In vivo- und In vitro- Untersuchungen haben gezeigt, dass CTLA-4 eine bedeutende Rolle in der Regulation von Selbsttoleranz einnimmt. Damit ist CTLA-4 ein entscheidendes Suspezeptibilitätsgen für Autoimmunität im Allgemeinen und könnte insbesondere für die T-Zell-vermittelten Autoimmunendokrinopathien bedeutend sein. Im Rahmen dieser Arbeit wurde nach einem Zusammenhang dieses Genortes mit Typ 1 Diabetes mellitus, Morbus Addison, Hashimoto-Thyreoiditis und Morbus Basedow in der kaukasischen Bevölkerung geforscht. Bei der Suche nach einer Assoziation zwischen CTLA-4 Polymorphismen und Typ 1 Diabetes mellitus zeigt sich eine signifikante Assoziation des -MH30 C/G - und -1661 A/G - Polymorphismus sowie des +49 A/G Exon 1- Polymorphismus des CTLA-4 Gens mit Typ 1 Diabetes mellitus. Beim Polymorphismus –MH30 C/G des CTLA-4 Gen zeigt sich ein signifikant gehäuftes Auftreten des Allels „G“ bei Patienten, die an Typ 1 Diabetes mellitus erkrankt sind. Dabei vermittelt das Allel „G“ bei der Allelfrequenzanalyse - wie auch bei der Genotypenanalyse ein signifikant erhöhtes Diabetes-Risiko. Das Allel „C“ hingegen ist signifikant seltener an Patienten und scheint somit eher protektiv gegen Typ 1 Diabetes mellitus zu wirken. Bei der Untersuchung des -1661 A/G Polymorphismus des CTLA-4 Gens bei Patienten mit Typ 1 Diabetes mellitus zeigt sich das Allel „A“ als krankheitsprädisponierend. Beim +49 A/G Polymorphismus führt die Substitution von Adenin zu Guanin an Position 49 des Exon 1 zu eine Threonin (Thr) /Alanin (Ala) Substitution am Kodon 17 der Signalpeptidsequenz. Die Untersuchung des +49 A/G Polymorphismus in der deutschen Population verdeutlicht, dass der Genotyp „G/G“ sowie das Allel „G“ (Alanin) krankheitsprädisponierend sind. Das Allel „A“ hat eine protektive Wirkung und tritt gehäuft in der Kontrollgruppe auf. Der – 319 – Promotorpolymorphismus zeigt keine Assoziation mit Typ 1 Diabetes mellitus. Die Haplotypanalyse -MH30/-1661 zeigt, dass der Haplotyp „CG“ bei Typ 1 Diabetes mellitus signifikant häufiger auftritt als in der Kontrollgruppe. Der Haplotyp –MH30G/- 1661G hat eine protektive Wirkung. Weiterhin vermittelt der Haplotyp -318/+49G ein erhöhtes Diabetes-Risiko, während der Haplotyp „C/A“ protektiv wirkt. Des Weiteren wurde die Assoziation der oben genannten vier Polymorphismen im CTLA-4 Gen mit der Autoimmunerkrankung Morbus Addison, untersucht. Beim - 1661A/G-Polymorphismus zeigt sich eine signifikant erhöhte Frequenz des Genotyp „A/A“ bei Patienten mit Morbus Addison. Der Genotyp „G/G“ sowie das Allel „G“ des Exon 1- Polymorphismus sind häufiger bei Patienten mit Morbus Addison als bei den Kontrollen und vermitteln somit ein erhöhtes Risiko an Morbus Addison zu erkranken. Im Gegensatz hierzu hat das Allel „A“ eine protektive Wirkung und tritt gehäuft in der Kontrollgruppe auf. Der –MH30 – Polymorphismus und der Promotorpolymorphismus des CTLA-4 Gen zeigen keine relevanten Assoziationen mit Morbus Addison in der deutschen Population. Die Haplotypanalyse der -319 und +49 Polymorphismen zeigt, dass der Haplotyp „C/A“ eine protektive Wirkung hat und daher das Risiko an Morbus Addison zu erkranken, senkt. Der Haplotyp – 319C/+49G birgt dagegen ein erhöhtes Erkrankungsrisiko. Bei Hashimoto-Thyreoiditis zeigte sich, dass der –MH30 C/G-Polymorphismus mit der Erkrankung assoziiert ist. Im Gegensatz zu den Ergebnissen bei Typ 1 Diabetes mellitus zeigt sich das Allel „C“ als krankheitsprädisponierend. Der Genotyp „C/C“ ist signifikant häufiger im Patientenkollektiv als in der Kontrollgruppe. Das Allel „G“ am Genort CTLA-4 –MH30 wirkt dagegen protekiv. Beim -1661 A/G – Polymorphismus treten beide homozygote Genotypen „A/A“ und „G/G“ im Patientenkollektiv häufiger auf als in der Kontrollgruppe. Der heterozygote Genotyp hingegen „A/G“ ist signifikant seltener in der Patientengruppe zu finden als in der Kontrollgruppe und hat somit eine protektive Wirkung. Die Untersuchung des -319 C/T – und des +49 A/G –Polymorphismus zeigt keine Assoziation mit Hashimoto – Thyreoiditis. Die Haplotypanalyse - MH30 / CTLA-4 – 1661 zeigt, dass die Haplotypen „CG“ und „GG“ eine krankheitsprädisponierende Wirkung haben und gehäuft im Patientenkollektiv auftreten als in der Kontrollgruppe. Der Haplotyp -319/ +49 weist bei Patienten mit Hashimoto- Thyreoiditis keine Assoziation auf. Analog zu den anderen oben aufgeführten Autoimmunendokrinopathien wurden auch vier CTLA-4 Polymorphismen auf ihre Korrelation mit Morbus Basedow hin untersucht. Es konnte hier gezeigt werden, dass die untersuchten Polymorphismen keinen wesentlichen Beitrag zur Vermittelung des genetischen Risikos für Morbus Basedow leisten. Die Korrelationsuntersuchung der Polymorphismen zeigte auch keine Assoziation der Haplotypen mit Morbus Basedow. Dies schließt jedoch das CTLA-4 Gen als ein Suspezeptibilitätsgen für Morbus Basedow nicht aus, zumal Assoziationen mit weiteren in dieser Arbeit nicht untersuchten CTLA-4 Polymorphismen bestehen können. Zusammengefasst wird durch die Untersuchung dieser Doktorarbeit die These unterstützt, dass das CTLA-4 Gen eine wichtige Rolle in der Entstehung der Autoimmunendokrinopathien einnimmt. Die hier untersuchten Polymorphismen des CTLA-4 Gens sind prädisponierende Faktoren für Autoimmunendokrinopathien, insbesondere für Typ 1 Diabetes mellitus, Morbus Addison und Hashimoto-Thyreoiditis.
In den letzten Jahren hat sich die subkutane Schweißdrüsensaugkürettage als Therapie der Wahl bei den operativen Therapieverfahren der Hyperhidrosis axillaris etabliert. In der vorliegenden Studie wurde der klinische Langzeiterfolg der subkutanen Schweißdrüsensaugkürettage unter Einbeziehung quantitativer Meßmethoden untersucht. Der durchschnittliche Nachbeobachtungszeitraum lag bei 31 Monaten. Die Datenerhebung wurde anhand der gravimetrisch ermittelten Schweißmenge präund postoperativ sowie anhand eines Fragebogens vorgenommen. 149 Patienten wurden angeschrieben, 27 waren zum Zeitpunkt der Nachuntersuchung nicht erreichbar, 25 Patienten konnten den Fragebogen ausfüllen aber nicht persönlich zur Nachuntersuchung erscheinen. Bei 97 Patienten konnte auch eine Nachuntersuchung durchgeführt werden. 77% der Patienten waren weiblich und 33% männlich. Der Altersdurchschnitt bei Erstmanifestation der Hyperhidrosis axillaris lag bei 20 Jahren, das Durchschnittsalter der Patienten bei der Operation betrug 29 Jahre. Die Anamnesedauer bis zur subkutanen Schweißdrüsensaugkürettage lag im Durchschnitt bei 7,91 Jahre. Bei 45% der Patienten konnte eine positive Familienanamnese ermittelt werden. In der vorliegenden Arbeit erfolgten gravimetrische Messungen prä- und postoperativ. Es zeigte sich eine signifikante Reduktion der Schweißproduktion um 54% von 271mg/5min an der rechten Axilla auf 103mg/5min und um 62% von 242mg/5min auf 90mg/5min an der linken Axilla. Es konnte zusätzlich eine Korrelation der von den Patienten subjektiv empfundenen Schweißmengenreduktion mit den gravimetrisch ermittelten Werten nachgewiesen werden. Langfristige Nebenwirkungen wurden nicht festgestellt. Es entstanden nur kleine OPNarben, die von keinem Patient als kosmetisch störend beurteilt wurde. Zusammenfassend zeigen die Ergebnisse dieser Arbeit, dass durch die subkutane Schweißdrüsensaugkürettage eine langfristige Reduktion der Schweißmengensekretion über 31 Monaten bei der Hyperhidrosis axillaris erzielt werden kann und somit mittels minimalinvasiver Chirurgie eine Krankheit erfolgreich behandelt werden kann, die die Lebensqualität der Patienten massiv einschränkt.
We report the case of a 62-year-old man with chronic pancreatitis who presented with increasing abdominal pain. Sonography, magnetic resonance imaging, contrast-enhanced computed tomography, and ultimately catheter angiography demonstrated a pancreatic pseudocyst that had eroded into the splenoportal venous confluence, mimicking an arterial aneurysm. The diagnostic was confirmed at the time of surgical treatment. This case demonstrates the use of imaging to diagnose complications of pancreatitis, and the difficulty of distinguishing an eroding pseudocyst from an arterial aneurysm.
Large-scale molecular profiling studies in recent years have shown that central nervous system (CNS) tumors display a much greater heterogeneity in terms of molecularly distinct entities, cellular origins and genetic drivers than anticipated from histological assessment. DNA methylation profiling has emerged as a useful tool for robust tumor classification, providing new insights into these heterogeneous molecular classes. This is particularly true for rare CNS tumors with a broad morphological spectrum, which are not possible to assign as separate entities based on histological similarity alone. Here, we describe a molecularly distinct subset of predominantly pediatric CNS neoplasms (n = 60) that harbor PATZ1 fusions. The original histological diagnoses of these tumors covered a wide spectrum of tumor types and malignancy grades. While the single most common diagnosis was glioblastoma (GBM), clinical data of the PATZ1-fused tumors showed a better prognosis than typical GBM, despite frequent relapses. RNA sequencing revealed recurrent MN1:PATZ1 or EWSR1:PATZ1 fusions related to (often extensive) copy number variations on chromosome 22, where PATZ1 and the two fusion partners are located. These fusions have individually been reported in a number of glial/glioneuronal tumors, as well as extracranial sarcomas. We show here that they are more common than previously acknowledged, and together define a biologically distinct CNS tumor type with high expression of neural development markers such as PAX2, GATA2 and IGF2. Drug screening performed on the MN1:PATZ1 fusion-bearing KS-1 brain tumor cell line revealed preliminary candidates for further study. In summary, PATZ1 fusions define a molecular class of histologically polyphenotypic neuroepithelial tumors, which show an intermediate prognosis under current treatment regimens.

In this study, it is demonstrated that moving sounds have an effect on the direction in which one sees visual stimuli move. During the main experiment sounds were presented consecutively at four speaker locations inducing left or rightward auditory apparent motion. On the path of auditory apparent motion, visual apparent motion stimuli were presented with a high degree of directional ambiguity. The main outcome of this experiment is that our participants perceived visual apparent motion stimuli that were ambiguous (equally likely to be perceived as moving left or rightward) more often as moving in the same direction than in the opposite direction of auditory apparent motion. During the control experiment we replicated this finding and found no effect of sound motion direction on eye movements. This indicates that auditory motion can capture our visual motion percept when visual motion direction is insufficiently determinate without affecting eye movements.
Background: Currently, there is inadequate evidence on which to base clinical management of neurotoxic snakebite envenoming, especially in the choice of initial antivenom dosage. This randomised controlled trial compared the effectiveness and safety of high versus low initial antivenom dosage in victims of neurotoxic envenoming.
Methodology/ Principal findings: This was a balanced, randomised, double-blind trial that was conducted in three health care centers located in the Terai plains of Nepal. Participants received either low (two vials) or high (10 vials) initial dosage of Indian polyvalent antivenom. The primary composite outcome consisted of death, the need for assisted ventilation and worsening/recurrence of neurotoxicity. Hourly evaluations followed antivenom treatment. Between April 2011 and October 2012, 157 snakebite victims were enrolled, of which 154 were analysed (76 in the low and 78 in the high initial dose group). Sixty-seven (43·5%) participants met the primary outcome definition. The proportions were similar in the low (37 or 48.7%) vs. high (30 or 38.5%) initial dose group (difference = 10·2%, 95%CI [-6·7 to 27·1], p = 0·264). The mean number of vials used was similar between treatment groups. Overall, patients bitten by kraits did worse than those bitten by cobras. The occurrence of treatment-related adverse events did not differ among treatment groups. A total of 19 serious adverse events occurred, including seven attributed to antivenom.
Conclusions: This first robust trial investigating antivenom dosage for neurotoxic snakebite envenoming shows that the antivenom currently used in Nepal performs poorly. Although the high initial dose regimen is not more effective than the low initial dose, it offers the practical advantage of being a single dose, while not incurring higher consumption or enhanced risk of adverse reaction. The development of new and more effective antivenoms that better target the species responsible for bites in the region will help improve future patients’ outcomes.

Das Ziel dieser Studie war der retrospektive Vergleich zweier fortschrittlicher robotergestützter Angiographie-Systeme in Bezug auf Strahlendosis und Bildqualität bei der Bildgebung im Rahmen der konventionellen transarteriellen Chemoembolisation (cTACE) von bösartigen Lebertumoren.
Dafür haben wir 106 Patienten (insgesamt 57 Frauen und 49 Männer; Durchschnittsalter 60 ± 11 Jahre), welche eine cTACE Therapie an einem der zwei Generationen von Roboter-Angiographieplattformen erhalten hatten, einbezogen. Die Patienten wurden in zwei Gruppen eingeteilt (n=53): Gruppe 1 (Behandlung am Gerät der ersten Generation) und Gruppe 2 (Behandlung am Gerät der zweiten Generation). Die Strahlendosis für die Fluoroskopie und die digitale Subtraktions-Angiographie (DSA) wurde zwischen den Angiographiegeräten der ersten bzw. zweiten Generation verglichen. Zu den besonderen Merkmalen des Systems der zweiten Generation- im Vergleich zum System der ersten Generation - gehörten ein verfeinertes kristallines Detektorsystem, zur verbesserten Rauschunterdrückung und eine fortschrittliche CARE-Filtersoftware, zur Senkung der Strahlendosis. Die Strahlendosis wurde mit einer herkömmlichen im Gerät verbauten Ionisationskammer gemessen. Die Bildqualität wurde von drei unabhängigen Radiologen anhand einer 5-Punkt-Likert-Skala bewertet. Beide Gruppen waren in Bezug auf Anzahl und Lage der Läsionen sowie Körpergewicht, BMI-Werte und anatomische Varianten der versorgenden Leberarterien vergleichbar (alle p > 0,05).
Das Dosisflächenprodukt (DAP) für die Fluoroskopie war in Gruppe 2 signifikant niedriger (1,4 ± 1,1 Gy·cm2) als in Gruppe 1 (2,8 ± 3,4 Gy·cm2; p = 0,001). Für DSA war DAP in Gruppe 2 (2,2 ± 1,2 Gy·cm2) signifikant niedriger (p = 0,003) gegenüber Gruppe 1 (4,7 ± 2,3 Gy·cm2). Die Ergebnisse für die DSA-Bildqualität (IQ) zeigten signifikante Verbesserungen für Gruppe 2 um 30% im Vergleich zu Gruppe 1 (p = 0,004). In der Fluoroskopie waren die Werte für den IQ in Gruppe 2 76% höher als in Gruppe 1 (p = 0,001). Eine gutes bis sehr gutes Inter-rater-agreement mit Kappa- Fleiss-Koeffizienten von κ = 0,75 für Gruppe 1 und κ = 0,74 für Gruppe 2 wurde erreicht.
Zusammenfassend ließ sich feststellen, dass die neueste Generation der robotergestützten Angiographiegeräte im Zusammenhang von cTACE der Leber eine erhebliche Reduzierung der Strahlendosis bei gleichzeitiger Verbesserung der Bildqualität in der Fluoroskopie und DSA-Bildführung ermöglicht.
In context of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), patients with certain comorbidities and high age, as well as male sex are considered to represent the risk group for severe course of disease. Corona-virus disease 2019 (COVID-19) typical CT-patterns include bilateral, peripheral ground glass opacity (GGO), septal thickening, bronchiectasis, consolidation as well as associated pleural effusion. We report a 77-year-old heart transplanted patient with confirmed COVID-19 infection and coronary heart disease, diabetes type II and other risk factors. Notably, only slight clinical symptoms were reported and repeated computed tomography (CT) scans showed an atypical course of CT findings during his hospitalization.
Case report of rare congenital cardiovascular anomalies associated with truncus arteriosus type 2

Truncus arteriosus (TA) is a very rare congenital anomaly with complex cardiovascular anatomy and high lethality also due to severe associated anatomical variants and pathologies. As TA has a massive impact on the survival of a newborn and usually has to be surgically treated. Thus, it is of high importance to understand this congenital cardiovascular disease and associated complications, to improve life expectancy and outcome of these patients. We recently came across a newborn female patient with a rare complex case of persistent TA type 2 associated with further complex cardiovascular anomalies, who received a contrast enhanced CT scan on the 3 rd day post-partum, showing complex cardiovascular abnormalities that were ultimately incompatible with life.
Vaccination represents one of the fundamentals in the fight against SARS-CoV-2. Myocarditis has been reported as a rare but possible adverse consequence of different vaccines, and its clinical presentation can range from mild symptoms to acute heart failure. We report a case of a 29-year-old man who presented with fever and retrosternal pain after receiving SARS-CoV-2 vaccine. Cardiac magnetic resonance imaging and laboratory data revealed typical findings of acute myocarditis.
Bis einschließlich 10. Januar 2006 infizierten sich in Asien rund 150 Menschen mit dem Erreger der Vogelgrippe H5N1. In sechs Ländern (Kambodscha, China, Indonesien, Thailand, Vietnam und Türkei) verstarben an der “Hühnergrippe” rund 80 Patienten. Eine Übertragung von Mensch zu Mensch scheint in Einzelfällen möglich. Eine Pandemie hat der Erreger bisher nicht ausgelöst: Er wurde nicht (effektiv) von Mensch zu Mensch übertragen.
Aktuell erscheint aber eine Ausweitung der Hühnergrippe auch in Europa denkbar. Meldungen aus Rumänien im Oktober 2005 lassen eine Ausbreitung des H5N1-Erregers bei Wasservögeln vermuten. Jetzt (Stand Januar 2006) wurden auch aus der Türkei mehrere Infektionen des Menschen, davon drei Todesfälle, bekannt.
Sorge bereitet Experten die Möglichkeit eines genetischen “Reassortment” durch eine gleichzeitige Doppel-Infektion eines Wirtes (Mensch, Schwein) mit humanen und aviären Influenza-A-Viren-Erregern. Der neue Subtyp könnte bei passender Adaption an die menschlichen Zellen zu einer neuen Pandemie führen.
Infektionen mit dem Respiratory Syncytial Virus (RSV)sind weltweit die bedeutendsten Atemwegserkrankungenim Säuuglings- und Kindesalter. Die RS-Viren werdend urch Schmierinfektionen und Aerosole übertragen, der Mensch ist das einzige Erregerreservoir. Im Säuglings-und Kleinkindalter finden gehäuft RSV-Infektionen statt. Mit zwei Jahren sind bereits 95% der Kinder seropositiv. Maternale Antikörper gewährleisten im Säuuglingsalterkeinen ausreichenden Nestschutz. Es ist von keiner sicheren Immunität auszugehen, daher sind Reinfektionen die Regel. Der Haüfigkeitsgipfel der RSV-Infektionenliegt in den Winter- und Frühlingsmonaten. Frühgeborene, immundefiziente und immunsupprimierte Patienten können das Virus mehrere Wochen ausscheiden. RSV-Infektionen verursachen zumeist Bronchitis, Bronchiolitis oder Pneumonie. Die Methode der Wahl ist der Erregernachweis über eine Virusisolierung in der Zellkultur im akuten Erkrankungsfall. Benötigt wird Nasenspülwasser oder ein tiefer Rachenabstrich. Auf einen schnellen Transport unter gekühlten Bedingungen ist zu achten (48C). Die Antikörpernachweise (Serologie) sind die Methode der Wahl für die epidemiologischen Auswertungen und weniger für die Akutdiagnostik geeignet. Nachdem Infektionsschutzgesetz (IfSG) § 6 Abs. 3 sind dem Gesundheitsamt gehäuft auftretende RSV-Infektionen zu melden. Die Therapie erfolgt symptomatisch; in schweren Fällen kann Ribavirin als Aerosol eingesetzt werden. Eine passive Immunisierung mit humanen Antikörpern gegen RSV kann bei Kindern mit erhöhtem Infektionsrisiko i.v. verabreicht werden (RespiGam). Auch sind monoklonale Antikörper gegen RSV (Palivizumab) prophylaktisch wirksam.
Introduction: Studies of vocational ballet students are sparce. In particular, there is a lack of gender comparisons. The aim of the present study, therefore, was to give a musculoskeletal and sociodemographic description of the typical vocational ballet student in gender comparison. Methods: In this study, n = 414 female and n = 192 male students of the John Cranko School (JCS), aged between 5 and 22 years (Mean ± SD: 13.9 ± 3.5), were examined by an experienced orthopedist and dance physician. Results: Males started ballet (5.8/8.2 years, p < 0.001) and training at later age than females (13.5/14.6 years, p < 0.05). There was a high prevalence of low body weight among both sexes; however, particularly among female participants (58.4/16.2%, p < 0.001). Both sexes showed a large external rotation of the hip (f/m: 59/62°, p < 0.001), a large turnout (f/m: 82/86°, p < 0.01), high values for plantarflexion of the ankle joint (f/m: 72/68°, p < 0.001) and dorsiflexion of the metatarsophalangeal joint of the big toe (f/m: 90/87°, p < 0.001). Discussion: Differences in ballet-specific characteristics between genders (f/m) are converging and are smaller than described in the past. The particularly high prevalence of low body weight among students in the vocational training sector, particularly among females, highlights the need for deeper diagnostic investigation.
A point mutation in the Ncr1 signal peptide impairs the development of innate lymphoid cell subsets

NKp46 (CD335) is a surface receptor shared by both human and mouse natural killer (NK) cells and innate lymphoid cells (ILCs) that transduces activating signals necessary to eliminate virus-infected cells and tumors. Here, we describe a spontaneous point mutation of cysteine to arginine (C14R) in the signal peptide of the NKp46 protein in congenic Ly5.1 mice and the newly generated NCRB6C14R strain. Ly5.1C14R NK cells expressed similar levels of Ncr1 mRNA as C57BL/6, but showed impaired surface NKp46 and reduced ability to control melanoma tumors in vivo. Expression of the mutant NKp46C14R in 293T cells showed that NKp46 protein trafficking to the cell surface was compromised. Although Ly5.1C14R mice had normal number of NK cells, they showed an increased number of early maturation stage NK cells. CD49a+ILC1s were also increased but these cells lacked the expression of TRAIL. ILC3s that expressed NKp46 were not detectable and were not apparent when examined by T-bet expression. Thus, the C14R mutation reveals that NKp46 is important for NK cell and ILC differentiation, maturation and function.
Isolated complex I deficiency is a common biochemical phenotype observed in pediatric mitochondrial disease and often arises as a consequence of pathogenic variants affecting one of the ∼65 genes encoding the complex I structural subunits or assembly factors. Such genetic heterogeneity means that application of next-generation sequencing technologies to undiagnosed cohorts has been a catalyst for genetic diagnosis and gene-disease associations. We describe the clinical and molecular genetic investigations of four unrelated children who presented with neuroradiological findings and/or elevated lactate levels, highly suggestive of an underlying mitochondrial diagnosis. Next-generation sequencing identified bi-allelic variants in NDUFA6, encoding a 15 kDa LYR-motif-containing complex I subunit that forms part of the Q-module. Functional investigations using subjects’ fibroblast cell lines demonstrated complex I assembly defects, which were characterized in detail by mass-spectrometry-based complexome profiling. This confirmed a marked reduction in incorporated NDUFA6 and a concomitant reduction in other Q-module subunits, including NDUFAB1, NDUFA7, and NDUFA12. Lentiviral transduction of subjects’ fibroblasts showed normalization of complex I. These data also support supercomplex formation, whereby the ∼830 kDa complex I intermediate (consisting of the P- and Q-modules) is in complex with assembled complex III and IV holoenzymes despite lacking the N-module. Interestingly, RNA-sequencing data provided evidence that the consensus RefSeq accession number does not correspond to the predominant transcript in clinically relevant tissues, prompting revision of the NDUFA6 RefSeq transcript and highlighting not only the importance of thorough variant interpretation but also the assessment of appropriate transcripts for analysis.
Leigh syndrome is one of the most common neurological phenotypes observed in pediatric mitochondrial disease presentations. It is characterized by symmetrical lesions found on neuroimaging in the basal ganglia, thalamus, and brainstem and by a loss of motor skills and delayed developmental milestones. Genetic diagnosis of Leigh syndrome is complicated on account of the vast genetic heterogeneity with >75 candidate disease-associated genes having been reported to date. Candidate genes are still emerging, being identified when “omics” tools (genomics, proteomics, and transcriptomics) are applied to manipulated cell lines and cohorts of clinically characterized individuals who lack a genetic diagnosis. NDUFAF8 is one such protein; it has been found to interact with the well-characterized complex I (CI) assembly factor NDUFAF5 in a large-scale protein-protein interaction screen. Diagnostic next-generation sequencing has identified three unrelated pediatric subjects, each with a clinical diagnosis of Leigh syndrome, who harbor bi-allelic pathogenic variants in NDUFAF8. These variants include a recurrent splicing variant that was initially overlooked due to its deep-intronic location. Subject fibroblasts were found to express a complex I deficiency, and lentiviral transduction with wild-type NDUFAF8-cDNA ameliorated both the assembly defect and the biochemical deficiency. Complexome profiling of subject fibroblasts demonstrated a complex I assembly defect, and the stalled assembly intermediates corroborate the role of NDUFAF8 in early complex I assembly. This report serves to expand the genetic heterogeneity associated with Leigh syndrome and to validate the clinical utility of orphan protein characterization. We also highlight the importance of evaluating intronic sequence when a single, definitively pathogenic variant is identified during diagnostic testing.
Background: Methotrexate (MTX) remains the anchor drug in rheumatoid arthritis (RA) treatment, but is poorly tolerated or contraindicated in some patients. There is a wealth of data supporting the use of abatacept in combination with MTX, but data on alternative conventional synthetic disease-modifying antirheumatic drug (csDMARD) combinations with abatacept are scarce.
Methods: In this post-hoc exploratory analysis, efficacy and safety data were extracted from abatacept RA studies in which combination with csDMARDs other than MTX was permitted: three interventional trials (ATTAIN, ASSURE, and ARRIVE) and one real-world study (ACTION). Patients with moderate-to-severe RA received abatacept in combination with MTX, hydroxychloroquine, sulfasalazine, azathioprine, or leflunomide for 6 months to 2 years according to the study design. Change from baseline in physical function (Health Assessment Questionnaire—Disability Index (HAQ-DI); all studies) and 28-joint Disease Activity Score (C-reactive protein) (DAS28 (CRP); ATTAIN, ARRIVE, and ACTION), American College of Rheumatology response rates (ATTAIN), and safety were assessed for individual and pooled csDMARD combinations for each trial. A meta-analysis was also performed on pooled data for HAQ-DI and DAS28 (CRP) across interventional trials.
Results: Across all four studies, 731 patients received abatacept plus one non-MTX csDMARD (hydroxychloroquine n = 152; sulfasalazine n = 123; azathioprine n = 59; and leflunomide n = 397) and 2382 patients received abatacept plus MTX. Mean changes from baseline in HAQ-DI scores for abatacept plus MTX (all csDMARDs pooled) vs abatacept plus a non-MTX csDMARD were –0.54 vs –0.44 (ATTAIN), –0.43 vs –0.43 (ASSURE), and –0.39 vs –0.36 (ARRIVE). Mean changes from baseline in DAS28 (CRP) and ACR response rates were also similar with abatacept plus MTX or non-MTX csDMARDs. Data for individual non-MTX csDMARDs (pooled across studies) and real-world data were consistent with these findings. Rates of treatment-related adverse events and serious adverse events, respectively, for abatacept plus one non-MTX csDMARD vs abatacept plus MTX were 35.7% vs 41.7% and 2.4% vs 2.3% (ATTAIN), 58.0% vs 55.9% and 4.2% vs 1.7% (ASSURE), and 38.1% vs 44.3% and 0.6% vs 2.9% (ARRIVE).
Conclusions: Abatacept in combination with non-MTX csDMARDs is clinically effective and well tolerated in patients with moderate-to-severe RA, providing similar benefits to those seen with abatacept plus MTX.
Compartmentalization of immune response and microbial translocation in decompensated cirrhosis

Background: Acquired dysfunctional immunity in cirrhosis predisposes patients to frequent bacterial infections, especially spontaneous bacterial peritonitis (SBP), leading to systemic inflammation that is associated with poor outcome. But systemic inflammation can also be found in the absence of a confirmed infection. Detection of bacterial DNA has been investigated as a marker of SBP and as a predictor of prognosis. Data is, however, contradictory. Here we investigated whether levels of IL-6 and IL-8 putatively produced by myeloid cells in ascites are associated with systemic inflammation and whether inflammation depends on the presence of specific bacterial DNA.
Methods and Materials: We enrolled 33 patients with decompensated liver cirrhosis from whom we collected paired samples of blood and ascites. IL-6 and IL-8 were measured in serum samples of all patients using ELISA. In a subset of 10 representative patients, bacterial DNA was extracted from ascites and whole blood, followed by 16S rRNA gene amplicon sequencing.
Results: There were significantly higher levels of IL-6 in ascites fluid compared to blood samples in all patients. Interestingly, IL-6 levels in blood correlated tightly with disease severity and surrogates of systemic inflammation, while IL-6 levels in ascites did not. Moreover, patients with higher blood CRP levels showed greater SBP prevalence compared to patients with lower levels, despite similar positive culture results. Bacterial richness was also significantly higher in ascites compared to the corresponding patient blood. We identified differences in microbial composition and diversity between ascites and blood, but no tight relationship with surrogates of systemic inflammation could be observed.
Discussion: In decompensated cirrhosis, markers of systemic inflammation and microbiota composition seem to be dysregulated in ascites and blood. While a relationship between systemic inflammation and microbiota composition seems to exist in blood, this is not the case for ascites in our hands. These data may suggest compartmentalization of the immune response and interaction of the latter with the microbiota especially in the blood compartment.
White matter microstructural changes and episodic memory disturbances in late-onset bipolar disorder

Background: Bipolar disorder (BD) has been associated with distributed network disruption, but little is known on how different clinical subtypes, particularly those with an earlier and later onset of disease, are related to connectivity changes in white matter (WM) tracts.
Methods: Diffusion tensor imaging (DTI) and volumetric measures were carried out in early-onset bipolar patients [(EOD) (n = 16)], late-onset bipolar disorder [(LOD)(n = 14)] and healthy controls (n = 32). We also computed ROI analysis of gray matter (GM) and white matter (WM) volumes using the regions with significant group differences in the DTI parameters. Cognitive and behavior measurements were analyzed between groups.
Results: Lower fraction of anisotropy (FA) in the right hemisphere comprising anterior thalamic radiation, fornix, posterior cingulate, internal capsule, splenium of corpus callosum was observed in the LOD in comparison with EOD; additionally, lower FA was also found in the LOD in comparison with healthy controls, mostly in the right hemisphere and comprising fibers of the splenium of the corpus callosum, cingulum, superior frontal gyrus and posterior thalamic radiation; LOD also showed worse episodic memory performance than EOD; no statistical significant differences between mood symptoms, WM and GM volumes were found between BD groups.
Conclusion: Even after correcting for age differences, LOD was associated with more extensive WM microstructural changes and worse episodic memory performance than EOD; these findings suggest that changes in the WM fiber integrity may be associated with a later presentation of BD, possibly due to mechanisms other than neuroprogression. However, these findings deserve replication in larger, prospective, studies.

The interpretation of diffusion tensor imaging (DTI) eigenvectors, known as multiple indices, may provide new insights into the main pathological models supporting primary or secondary patterns of WM disruption in AD, the retrogenesis, and Wallerian degeneration models, respectively. The aim of this review is to analyze the current literature on the contribution of DTI multiple indices to the understanding of AD neuropathology, taking the retrogenesis model as a reference for discussion. A systematic review using MEDLINE, EMBASE, and PUBMED was performed. Evidence suggests that AD evolves through distinct patterns of WM disruption, in which retrogenesis or, alternatively, the Wallerian degeneration may prevail. Distinct patterns of WM atrophy may be influenced by complex interactions which comprise disease status and progression, fiber localization, concurrent risk factors (i.e., vascular disease, gender), and cognitive reserve. Alzheimeŕs disease (AD) represents the most prevalent neurodegenerative disorder that causes cognitive decline in old age. In its early stages, AD is associated with microstructural abnormalities in white matter (WM). In the current study, multiple indices of diffusion tensor imaging (DTI) and brain volumetric measurements were employed to comprehensively investigate the landscape of AD pathology. The sample comprised 58 individuals including cognitively normal subjects (controls), amnestic mild cognitive impairment (MCI) and AD patients. Relative to controls, both MCI and AD subjects showed widespread changes of anisotropic fraction (FA) in the corpus callosum, cingulate and uncinate fasciculus. Mean diffusivity and radial changes were also observed in AD patients in comparison with controls. After controlling for the gray matter atrophy the number of regions of significantly lower FA in AD patients relative to controls was decreased; nonetheless, unique areas of microstructural damage remained, e.g., the corpus callosum and uncinate fasciculus. Despite sample size limitations, the current results suggest that a combination of secondary and primary degeneration occurrs in MCI and AD, although the secondary degeneration appears to have a more critical role during the stages of disease involving dementia.
CD8+ T cells are key players in immunity against intracellular infections and tumors. The main cytokine associated with these protective responses is interferon-γ (IFN-γ), whose production is known to be regulated at the transcriptional level during CD8+ T cell differentiation. Here we found that microRNAs constitute a posttranscriptional brake to IFN-γ expression by CD8+ T cells, since the genetic interference with the Dicer processing machinery resulted in the overproduction of IFN-γ by both thymic and peripheral CD8+ T cells. Using a gene reporter mouse for IFN-γ locus activity, we compared the microRNA repertoires associated with the presence or absence of IFN-γ expression. This allowed us to identify a set of candidates, including miR-181a and miR-451, which were functionally tested in overexpression experiments using synthetic mimics in peripheral CD8+ T cell cultures. We found that miR-181a limits IFN-γ production by suppressing the expression of the transcription factor Id2, which in turn promotes the Ifng expression program. Importantly, upon MuHV-4 challenge, miR-181a-deficient mice showed a more vigorous IFN-γ+ CD8+ T cell response and were able to control viral infection significantly more efficiently than control mice. These data collectively establish a novel role for miR-181a in regulating IFN-γ–mediated effector CD8+ T cell responses in vitro and in vivo.
Introduction Complex psychopathological and behavioral symptoms, such as delusions and aggression against care providers, are often the primary cause of acute hospital admissions of elderly patients to emergency units and psychiatric departments. This issue resembles an interdisciplinary clinically highly relevant diagnostic and therapeutic challenge across many medical subjects and general practice. At least 50% of the dramatically growing number of patients with dementia exerts aggressive and agitated symptoms during the course of clinical progression, particularly at moderate clinical severity. Methods Commonly used rating scales for agitation and aggression are reviewed and discussed. Furthermore, we focus in this article on benefits and limitations of all available data of anticonvulsants published in this specific indication, such as valproate, carbamazepine, oxcarbazepine, lamotrigine, gabapentin and topiramate. Results To date, most positive and robust data are available for carbamazepine, however, pharmacokinetic interactions with secondary enzyme induction limit its use. Controlled data of valproate do not seem to support the use in this population. For oxcarbazepine only one controlled but negative trial is available. Positive small series and case reports have been reported for lamotrigine, gabapentin and topiramate. Conclusions So far, data of anticonvulsants in demented patients with behavioral disturbances are not convincing. Controlled clinical trials using specific, valid and psychometrically sound instruments of newer anticonvulsants with a better tolerability profile are mandatory to verify whether they can contribute as treatment option in this indication.
Qualitätsmanagement in der Arztpraxis : Begleitung eines Managementprojekts im Ärztenetz MQLD

Im Jahr 2000 wird im MQLD (Medizinisches Qualitätsnetz Langen-Dreieich), einem Ärztenetz in Südhessen, das Modellprojekt "Personalentwicklung und Mitarbeiterschulung" auf der Basis der DIN EN ISO 9001:2000 entwickelt, das Arbeitsbereiche wie Praxismanagement, Patientenservice und Kommunikation mit den Patienten durch themenbezogene Trainingseinheiten, aufbauend auf den Ergebnissen einer fragebogenbasierten Patienten- und Mitarbeiterbefragung, verbessern soll. Das Projekt wird von der Kassenärztlichen Vereinigung (KV) Hessen mitfinanziert und von der Firma Medizin-QM durchgeführt. Es ist als "dynamisches Modell" konzipiert und damit vergleichbar mit anderen QM-Systemen (QM = Qualitätsmanagement) wie EPA (Europäisches Praxisassessment) oder dem System der Berlin-Chemie. Ziel der vorliegenden Untersuchung ist es, das QM-Projekt als Beginn einer dynamischen Entwicklung darzustellen, die Ergebnisse der einzelnen Schritte zu diskutieren und die Bedeutung des Projekts für das Umfeld von QM in der ambulanten Krankenversorgung zu erörtern. • Der Projektplan dient als Grundlage. Er definiert grundlegende Fragen zu Bearbeitungsobjekt, Projektzielen, Zusammenhang zwischen Projekt und Qualitätspolitik der Einrichtung, Evaluation, Verantwortlichkeit, Kooperation, Zeitplan, Logistik, Ablauf, Dokumentation und Integration der Ergebnisse in den Alltag. Die Kontinuitätseinführung von QM fehlt im Plan des MQLD-Projekts. Dadurch bleiben Projektziele wie die systematische Verbesserung der Netzkommunikation und die kontinuierliche Steigerung der Mitarbeiterleistung unerreicht. • Ein Projekt muss an die gegebene Infrastruktur angepasst werden. Das MQLDProjekt erreicht dies durch die Erstellung des Praxisprofils und zum Teil mittels der oben angeführten Ergebnisse der Ärzte-, Mitarbeiter- und Patienten-Befragung. • Eine Teilnahmequote von 17% der Praxen (9 von 53) ist zu gering. Die Quote kann in zukünftigen Projekten erhöht werden durch die Setzung positiver Teilnahmeanreize wie zum Beispiel Bonitätsregelungen, durch die Überzeugung, dass QM zur Verringerung des Arbeitsaufwands führt und durch optimale Information potentieller Projektteilnehmer. • Die Teilnahmequote der Mitarbeiter liegt mit 66% (31 von 47) im akzeptablen Bereich. Sie korreliert eng mit der Motivationsfähigkeit des Arztes. QM muss als Unternehmenskultur gelebt und Führungsaufgabe werden. • Die Zahl der teilnehmenden Patienten liegt mit 528 (verteilt auf acht Praxen) im unteren Akzeptanzbereich einer Stichprobengröße. In mindestens zwei Praxen ist die Patientenzahl mit 29 beziehungsweise 36 aber deutlich zu gering. Die Auswahl einer ausreichend großen und möglichst repräsentativen Patientenstichprobe von mindestens 50 bis 100 Patienten aus jeder Praxis und die Verteilung der Fragebögen müssen in Zukunft systematisch erfolgen, das heißt beispielsweise durch ein fortlaufende Ausgabe der Bögen inklusive eines frankierten Rückumschlags im Rahmen eines persönlichen Gesprächs. • Aufgrund besserer Vergleichbarkeit der Ergebnisse, insgesamt geringeren Zeit- und Organisationsaufwands, besserer Standardisierungsmöglichkeiten und wegen der Möglichkeit der Bearbeitung größerer Zahlen sind Fragebögen als quantitative Erhebungsinstrumente qualitativen Erhebungsinstrumenten wie beispielsweise Focus-Group-Discussions vorzuziehen. Die mangelhafte Erfassung unterrepräsentierter Gruppen, die Informationsfilterung und Praxisferne durch schriftliche Meinungsäußerung und die zeitlich verzögerte Rückmeldung müssen dafür in Kauf genommen werden. Fragebögen müssen dabei valide, reliabel, diskriminierungsfähig und einheitlich sein. Die eingesetzten Bögen erfüllen diesen Anspruch nicht. So ist auch der anzustrebende Vergleich der Ergebnisse mit externen Daten (Querschnittstudie) nicht möglich. • Mitarbeiterbeteiligung in Form von Befragung, Beurteilung und Schulung mit Fokussierung auf die Schwerpunkte Weiterbildung und Organisation der Arbeit ist essentiell für eine erfolgreiche Personalführung und führt zu qualitätsbewußten Mitarbeitern. Sie ist eines der Schlüsselelemente des modernen QM. Die Beurteilungsfähigkeit der Ärzte muss vor einer Mitarbeiterbeurteilung sichergestellt werden. • Eine Patientenbefragung ist als Teil des QM-Schlüsselelements ´Kunden´- Orientierung ein essentieller Bestandteil eines Projekts. • Die Schulungsveranstaltungen werden an die spezifischen Gegebenheiten und an die Befragungsergebnisse angepasst. Geschult werden die Bereiche Praxismanagement, Patientenservice und Marketing, Kommunikation mit den Patienten, Personalbeurteilung und -entwicklung und Netzkommunikation. • Die Durchführung der Schulungsveranstaltungen wird von den Teilnehmern für ausreichend bis mangelhaft befunden. Die angekündigten Lehrmethoden werden nicht eingesetzt. Die didaktische Qualität der Ausbilder ist daher sicherzustellen. • Im Vergleich mit anderen QM-Systemen zeigen sich Kompatibilitäten von Grundkonzepten, Themenschwerpunkten und einzelnen Bausteinen. Validierung und Zertifizierung des MQLD-Projekts sind jedoch unzureichend. Abschließend ist zu betonen, dass es sich beim QM-Projekt des MQLD um ein Modell handelt, das für die künftige QM-Arbeit in der Arztpraxis weiterentwickelt und optimiert werden muss. Es stellt lediglich die Basis der Entwicklung weiterer QM-Projekte dar.

For many members of the forensic community, insects still have an exotic status. This may be one reason why forensic entomology, the analysis of insect evidence for forensic and legal purposes, has not yet achieved the significance it deserves in forensic sciences. The present special issue may help to change that. ...
Recent reports have shown a dramatic loss in insect species and biomass. Since forensic entomology relies on the presence of insects, the question is whether this decline effects the discipline. The present review confirms that numerous studies document insect population declines or even extinction, despite the fact that the rates of decline and the methods used to demonstrate it are still much debated. However, with regard to a decline in necrophagous insects, there is little or only anecdotal data available. A hypothetical decrease in species diversity and population density in necrophagous insects could lead to a delayed colonization of dead bodies and a modified succession pattern due to the disappearance or new occurrence of species or their altered seasonality. Climate change as one of the drivers of insect decline will probably also have an impact on necrophagous insects and forensic entomology, leading to reduced flight and oviposition activity, modified growth rates and, therefore, an over- or underestimation of a minimum postmortem interval. Global warming with increased temperature and extreme weather requires a better understanding about necrophagous insect responses to environmental variations. Here, transgeneration effects in particular should be analysed in greater depth as this will help to understand rapid adaptation and plasticity in insects of forensic importance.
The estimation of the minimum time since death is one of the main applications of forensic entomology. This can be done by calculating the age of the immature stage of necrophagous flies developing on the corpse, which is confined to approximately 2–4 weeks, depending on temperature and species of the first colonizing wave of flies. Adding the age of the adult flies developed on the dead body could extend this time frame up to several weeks when the body is in a building or closed premise. However, the techniques for accurately estimating the age of adult flies are still in their beginning stages or not sufficiently validated. Here we review the current state of the art of analysing the aging of flies by evaluating the ovarian development, the amount of pteridine in the eyes, the degree of wing damage, the modification of their cuticular hydrocarbon patterns, and the increasing number of growth layers in the cuticula. New approaches, including the use of age specific molecular profiles based on the levels of gene and protein expression and the application of near infrared spectroscopy, are introduced, and the forensic relevance of these methods is discussed.
Background: Previous magnetic resonance imaging (MRI) research suggests that, prior to the onset of psychosis, high risk youths already exhibit brain abnormalities similar to those present in patients with schizophrenia.
Objectives: The goal of the present study was to describe the functional organization of endogenous activation in young adolescents who report auditory verbal hallucinations (AVH) in view of the “distributed network” hypothesis of psychosis. We recruited 20 young people aged 13–16 years who reported AVHs and 20 healthy controls matched for age, gender and handedness from local schools.
Methods: Each participant underwent a semi-structured clinical interview and a resting state (RS) neuroimaging protocol. We explored functional connectivity (FC) involving three different networks: 1) default mode network (DMN) 2) salience network (SN) and 3) central executive network (CEN). In line with previous findings on the role of the auditory cortex in AVHs as reported by young adolescents, we also investigated FC anomalies involving both the primary and secondary auditory cortices (A1 and A2, respectively).
Further, we explored between-group inter-hemispheric FC differences (laterality) for both A1 and A2. Compared to the healthy control group, the AVH group exhibited FC differences in all three networks investigated. Moreover, FC anomalies were found in a neural network including both A1 and A2. The laterality analysis revealed no between-group, inter-hemispheric differences.
Conclusions: The present study suggests that young adolescents with subclinical psychotic symptoms exhibit functional connectivity anomalies directly and indirectly involving the DMN, SN, CEN and also a neural network including both primary and secondary auditory cortical regions.
Reaktive Sauerstoffspezies (ROS) sind von großer Bedeutung für die Regulation des vaskulären Tonus und die intrazelluläre Signaltransduktion. Die ROS-produzierenden Enzyme der Gefäßwand sind bisher nur unzureichend charakterisiert. Wir untersuchten, ob eine ähnliche NADPH Oxidase wie in Leukozyten, mit den Untereinheiten p22phox und gp91phox auch in den Endothel- und glatten Muskelzellen der Gefäßwand existiert. Phorbol 12-myristate 13-acetat (PMA) aktiviert die leukozytäre NADPH Oxidase und steigert in endothelintakten Aortenringen die ROS-Produktion. PMA steigert auch die Radikalproduktion in Endothelzellen (EC); jedoch nicht in glatten Muskelzellen (SMC) und denudierten Aortenringen. ECs und SMCs exprimieren p22phox, während gp91phox nur in ECs nachgewiesen werden konnte. Mox1, ein Homolog von gp91phox konnte im Gegensatz dazu nur in SMCs und nicht in ECs gefunden werden. Die funktionelle Relevanz von gp91phox wurde mittels vergleichender Experimente mit Aorten-Segmenten von Wildtyp-, gp91phox-/--und eNOS-/--Mäusen analysiert. Die PMA-induzierte ROS-Produktion der Aortenring von Wildtyp und eNOS-/--Mäusen war ähnlich stark, während sie in den Aortenringen von gp91phox-/--Mäusen ausblieb. Der Radikalfänger Tiron verstärkte die endothelabhängige Relaxation in Aortensegmenten von Wildtyp-, jedoch nicht von gp91phox-/--Mäusen. Unsere Daten zeigen, dass ECs, ähnlich wie Leukozyten, eine gp91phox enthaltende NADPH Oxidase exprimieren. Dieses Enzym ist eine Hauptquelle arterieller ROS und beeinflusst die Bioverfügbarkeit von endothelialem NO.
