Background: Treatment of asthma does not always comply with asthma guidelines (AG). This may be rooted in direct or indirect resistance on the doctors’ and/or patients’ side or be caused by the healthcare system. To assess whether patients’ concepts and attitudes are really an implementation barrier for AG, we analysed the patients’ perspective of a “good asthma therapy” and contrasted their wishes with current recommendations.
Methods: Using a qualitative exploratory design, topic centred focus group (FG) discussions were performed until theoretical saturation was reached. Inclusion criteria were an asthma diagnosis and age above 18. FG sessions were recorded audio-visually and analysed via a mapping technique and content analysis performed according to Mayring (supported by MAXQDA®). Participants’ speech times and the proportion of time devoted to different themes were calculated using the Videograph System® and related to the content analysis.
Results: Thirteen men and 24 women aged between 20 and 77 from rural and urban areas attended five FG. Some patients had been recently diagnosed with asthma, others years previously or in childhood. The following topics were addressed: (a) concern about or rejection of therapy components, particularly corticosteroids, which sometimes resulted in autonomous uncommunicated medication changes, (b) lack of time or money for optimal treatment, (c) insufficient involvement in therapy choices and (d) a desire for greater empowerment, (e) suboptimal communication between healthcare professionals and (f) difficulties with recommendations conflicting with daily life. Primarily, (g) participants wanted more time with doctors to discuss difficulties and (h) all aspects of living with an impairing condition.
Conclusions: We identified some important patient driven barriers to implementing AG recommendations. In order to advance AG implementation and improve asthma treatment, the patients’ perspective needs to be considered before drafting new versions of AG. These issues should be addressed at the planning stage.

The administration of intravenous fluid to critically ill patients is one of the most common but also one of the most fiercely debated interventions in intensive care medicine. During the past decade, a number of important studies have been published which provide clinicians with improved knowledge regarding the timing, the type and the amount of fluid they should give to their critically ill patients. However, despite the fact that many thousands of patients have been enrolled in these trials of alternative fluid strategies, consensus remains elusive and practice is widely variable. Early adequate resuscitation of patients in shock followed by a restrictive strategy may be associated with better outcomes. Colloids such as modern hydroxyethyl starch are more effective than crystalloids in early resuscitation of patients in shock, and are safe when administered during surgery. However, these colloids may not be beneficial later in the course of intensive care treatment and should best be avoided in intensive care patients who have a high risk of developing acute kidney injury. Albumin has no clear benefit over saline and is associated with increased mortality in neurotrauma patients. Balanced fluids reduce the risk of hyperchloraemic acidosis and possibly kidney injury. The use of hypertonic fluids in patients with sepsis and acute lung injury warrants further investigation and should be considered experimental at this stage. Fluid therapy impacts relevant patient-related outcomes. Clinicians should adopt an individualized strategy based on the clinical scenario and best available evidence. One size does not fit all.
Mutations in blood stem cells do not necessarily have to result in leukaemia. It was only recently discovered that clones of mutated blood cells can be identified in many healthy people in old age. Nonetheless, clonal haematopoiesis, as scientists baptised this finding, is far from innocent. It is a formidable risk factor for cardiovascular diseases – on par with smoking, excess weight or high blood pressure. Why this is, is still a riddle to be solved.
Background: Increasing numbers of patients surviving malignant bone tumors around the knee joint have led to an increasing importance to investigate long-term results. This study assessed the long-term results of rotationplasty after resection of malignant bone tumors regarding functional outcome and quality of life to allow better comparison with other treatment options in bone cancer treatment.
Procedure: 60 participants who underwent rotationplasty due to bone cancer took part in this multicentric questionnaire- based study. The long-term functional outcome was measured by the Musculoskeletal tumor society score (MSTS) and the Tegner activity level scale. The health-related quality of life (HRQL) was assessed by using the Short Form Health Survey (SF-36).
Results: Patients treated with rotationplasty (median follow- up of 22 years, range 10–47 years) regained a high level of activity (median MSTS score of 24). Even a return to high level sports was possible (mean Tegner activity level scale of 4). Duration of follow-up did not influence the functional outcome. HRQL scores were comparable to the general German popula tion. Concerns of psychological problems due to the unusual appearance of the rotated foot have not been confirmed.
Conclusion: Rotationplasty can be a good alternative to en- doprosthetic replacement or amputation, either as primary surgery or as a salvage procedure. Especially for growing children and very active patients rotationplasty should be considered.
The substantia nigra is not the induction site in the brain of the neurodegenerative process that underlies Parkinson’s disease. Instead, the results of this semiquantitative study of 30 autopsy cases with incidental Lewy body pathology indicate that Parkinson’s disease in the brain commences with the formation of the very first immunoreactive Lewy neurites and Lewy bodies in noncatecholaminergic visceromotor neurons of the dorsal glossopharyngeusvagus complex, in projection neurons of the intermediate reticular zone, and in specific nerve cell types of the gain setting system (coeruleussubcoeruleus complex, caudal raphe nuclei, gigantocellular reticular nucleus), olfactory bulb, olfactory tract, and/or anterior olfactory nucleus in the absence of nigral involvement. The topographical parcellation of the nuclear grays described here is based upon known architectonic analyses of the human brain stem and takes into consideration the pigmentation properties of a few highly susceptible nerve cell types involved in Parkinson’s disease. In this sample and in all 58 ageand gendermatched controls, Lewy bodies and Lewy neurites do not occur in any of the known telencephalic Parkinson’s disease predilection sites (hippocampal formation, temporal mesocortex, proneocortical cingulate areas, amygdala, basal nucleus of Meynert, interstitial nucleus of the diagonal band of Broca, hypothalamic tuberomamillary nucleus).
Mesenchymal stem/stromal cells (MSCs) feature promising potential for cellular therapies, yet significant progress in development of MSC therapeutics and assays is hampered because of remarkable MSC heterogeneity in vivo and in vitro. This heterogeneity poses challenges for standardization of MSC characterization and potency assays as well as for MSC study comparability and manufacturing. This review discusses promising marker combinations for prospective MSC subpopulation enrichment and expansion, and reflects MSC phenotype changes due to environment and age. In order to address animal modelling in MSC biology, comparison of mouse and human MSC markers highlights current common ground of MSCs between species.
White matter abnormalities across different epilepsy syndromes in adults: an ENIGMA Epilepsy study

The epilepsies are commonly accompanied by widespread abnormalities in cerebral white matter. ENIGMA-Epilepsy is a large quantitative brain imaging consortium, aggregating data to investigate patterns of neuroimaging abnormalities in common epilepsy syndromes, including temporal lobe epilepsy, extratemporal epilepsy, and genetic generalized epilepsy. Our goal was to rank the most robust white matter microstructural differences across and within syndromes in a multicentre sample of adult epilepsy patients. Diffusion-weighted MRI data were analyzed from 1,069 non-epileptic controls and 1,249 patients: temporal lobe epilepsy with hippocampal sclerosis (N=599), temporal lobe epilepsy with normal MRI (N=275), genetic generalized epilepsy (N=182) and nonlesional extratemporal epilepsy (N=193). A harmonized protocol using tract-based spatial statistics was used to derive skeletonized maps of fractional anisotropy and mean diffusivity for each participant, and fiber tracts were segmented using a diffusion MRI atlas. Data were harmonized to correct for scanner-specific variations in diffusion measures using a batch-effect correction tool (ComBat). Analyses of covariance, adjusting for age and sex, examined differences between each epilepsy syndrome and controls for each white matter tract (Bonferroni corrected at p<0.001). Across “all epilepsies” lower fractional anisotropy was observed in most fiber tracts with small to medium effect sizes, especially in the corpus callosum, cingulum and external capsule. Less robust effects were seen with mean diffusivity. Syndrome-specific fractional anisotropy and mean diffusivity differences were most pronounced in patients with hippocampal sclerosis in the ipsilateral parahippocampal cingulum and external capsule, with smaller effects across most other tracts. Those with temporal lobe epilepsy and normal MRI showed a similar pattern of greater ipsilateral than contralateral abnormalities, but less marked than those in patients with hippocampal sclerosis. Patients with generalized and extratemporal epilepsies had pronounced differences in fractional anisotropy in the corpus callosum, corona radiata and external capsule, and in mean diffusivity of the anterior corona radiata. Earlier age of seizure onset and longer disease duration were associated with a greater extent of microstructural abnormalities in patients with hippocampal sclerosis. We demonstrate microstructural abnormalities across major association, commissural, and projection fibers in a large multicentre study of epilepsy. Overall, epilepsy patients showed white matter abnormalities in the corpus callosum, cingulum and external capsule, with differing severity across epilepsy syndromes. These data further define the spectrum of white matter abnormalities in common epilepsy syndromes, yielding new insights into pathological substrates that may be used to guide future therapeutic and genetic studies.
The apolipoprotein E4 (ApoE4) is an established risk factor for Alzheimer's disease (AD). Previous work has shown that this allele is associated with functional (fMRI) changes as well structural grey matter (GM) changes in healthy young, middle-aged and older subjects. Here, we assess the diffusion characteristics and the white matter (WM) tracts of healthy young (20-38 years) ApoE4 carriers and non-carriers. No significant differences in diffusion indices were found between young carriers (ApoE4+) and non-carriers (ApoE4-). There were also no significant differences between the groups in terms of normalised GM or WM volume. A feature selection algorithm (ReliefF) was used to select the most salient voxels from the diffusion data for subsequent classification with support vector machines (SVMs). SVMs were capable of classifying ApoE4 carrier and non-carrier groups with an extremely high level of accuracy. The top 500 voxels selected by ReliefF were then used as seeds for tractography which identified a WM network that included regions of the parietal lobe, the cingulum bundle and the dorsolateral frontal lobe. There was a non-significant decrease in volume of this WM network in the ApoE4 carrier group. Our results indicate that there are subtle WM differences between healthy young ApoE4 carriers and non-carriers and that the WM network identified may be particularly vulnerable to further degeneration in ApoE4 carriers as they enter middle and old age.
White matter microstructural changes and episodic memory disturbances in late-onset bipolar disorder

Background: Bipolar disorder (BD) has been associated with distributed network disruption, but little is known on how different clinical subtypes, particularly those with an earlier and later onset of disease, are related to connectivity changes in white matter (WM) tracts.
Methods: Diffusion tensor imaging (DTI) and volumetric measures were carried out in early-onset bipolar patients [(EOD) (n = 16)], late-onset bipolar disorder [(LOD)(n = 14)] and healthy controls (n = 32). We also computed ROI analysis of gray matter (GM) and white matter (WM) volumes using the regions with significant group differences in the DTI parameters. Cognitive and behavior measurements were analyzed between groups.
Results: Lower fraction of anisotropy (FA) in the right hemisphere comprising anterior thalamic radiation, fornix, posterior cingulate, internal capsule, splenium of corpus callosum was observed in the LOD in comparison with EOD; additionally, lower FA was also found in the LOD in comparison with healthy controls, mostly in the right hemisphere and comprising fibers of the splenium of the corpus callosum, cingulum, superior frontal gyrus and posterior thalamic radiation; LOD also showed worse episodic memory performance than EOD; no statistical significant differences between mood symptoms, WM and GM volumes were found between BD groups.
Conclusion: Even after correcting for age differences, LOD was associated with more extensive WM microstructural changes and worse episodic memory performance than EOD; these findings suggest that changes in the WM fiber integrity may be associated with a later presentation of BD, possibly due to mechanisms other than neuroprogression. However, these findings deserve replication in larger, prospective, studies.
Objective: Studies using diffusion tensor imaging (DTI) to investigate white matter (WM) microstructure in youths with conduct disorder (CD) have reported disparate findings. We investigated WM alterations in a large sample of youths with CD, and examined the influence of sex and callous-unemotional (CU) traits.
Method: DTI data were acquired from 124 youths with CD (59 female) and 174 typically developing (TD) youths (103 female) 9 to 18 years of age. Tract-based spatial statistics tested for effects of diagnosis and sex-by-diagnosis interactions. Associations with CD symptoms, CU traits, a task measuring impulsivity, and the impact of comorbidity, and age- and puberty-related effects were examined.
Results: Youths with CD exhibited higher axial diffusivity in the corpus callosum and lower radial diffusivity and mean diffusivity in the anterior thalamic radiation relative to TD youths. Female and male youths with CD exhibited opposite changes within the internal capsule, fornix, posterior thalamic radiation, and uncinate fasciculus. Within the CD group, CD symptoms and callous traits exerted opposing influences on corpus callosum axial diffusivity, with callous traits identified as the unique clinical feature predicting higher axial diffusivity and lower radial diffusivity within the corpus callosum and anterior thalamic radiation, respectively. In an exploratory analysis, corpus callosum axial diffusivity partially mediated the association between callous traits and impulsive responses to emotional faces. Results were not influenced by symptoms of comorbid disorders, and no age- or puberty-related interactions were observed.
Conclusion: WM alterations within the corpus callosum represent a reliable neuroimaging marker of CD. Sex and callous traits are important factors to consider when examining WM in CD.
Background: Peanuts are a member of the legume family (botanical family Leguminosae) and peanut allergies are the most common cause of food anaphylaxis in many countries. The prevalence of peanut allergy is increasing.
Methods: Experts from Germany and Austria performed a standardized literature search and published their consensus recommendations in a White Paper on Peanut Allergy, which this care pathway is based upon, thus, providing a comprehensive diagnosis and treatment algorithm.
Results: The most important diagnostic key elements include a detailed clinical medical history, evidence of peanut-specific sensitization by means of skin prick testing and/or in vitro determination of the peanut (extract)-specific IgE and/or the molecular component diagnostics (most important Ara h 2-specific IgE, sometimes also Ara h1-, 3-, 6-, 8- and 9-specific IgE) as well as the gold standard, the double-blind, placebo-controlled food challenge. The diagnostic algorithms were created for the following constellations: Suspected primary peanut allergy with a clear history of systemic immediate-type reaction, suspected primary peanut allergy with questionable symptoms, suspected secondary (possibly pollen-associated) peanut allergy with a history of solely oropharyngeal symptoms and incidental finding of sensitization and no peanut ingestion so far.
Conclusions: After established diagnosis the standard of care is counseling to avoid peanut contact and prescription of emergency medications (oral antihistamines, oral steroids, inhaled β2-agonists, injectable intramuscular epinephrine) as needed. Instruction on the use of these emergency medications should be provided. A preparation for oral immunotherapy (OIT) for 4 to 17 years old peanut allergic children/ adolescents has been recently approved by the regulatory authorities. OIT for peanut allergy shows high efficacy and an acceptable safety profile, improves quality of life, and health economic aspects. Thus it offers a therapeutic option for peanut allergic children and adolescents.

The current management of a primary IgE-mediated peanut allergy consists of the two basic pillars “exposure prophylaxis” with avoidance of the allergen and “emergency therapy” with short-term treatment of an acute allergic reaction after accidental ingestion. Accidental reactions are common despite attempted avoidance. The severity of an allergic or even anaphylactic reaction after accidental ingestion is difficult to assess prior to reaction. In addition, reaction thresholds may vary depending on the accompanying augmentation factor. Therefore, every peanut allergic patient should receive individual dietary counseling as well as instructions for the use of the emergency kit and a structured patient education program (anaphylaxis group training), if necessary. For the first time, since fall 2021 a causal treatment option with a drug for oral immunotherapy will now be available for 4‑ to 17-year-old peanut-allergic children and adolescents. The oral immunotherapy with peanut protein as defatted powder of Arachis hypogaea L., semen (peanuts) leads to desensitization with a good efficacy record and an acceptable safety profile. Other treatment options with different therapeutic approaches are also under development and will probably expand the range for treatment in the coming years.

Current theories of schizophrenia (ScZ) posit that the symptoms and cognitive dysfunctions arise from a dysconnection syndrome. However, studies that have examined this hypothesis with physiological data at realistic time scales are so far scarce. The current study employed a state-of-the-art approach using Magnetoencephalography (MEG) to test alterations in large-scale phase synchronization in a sample of n = 16 chronic ScZ patients, 10 males and n = 19 healthy participants, 10 males, during a perceptual closure task. We identified large-scale networks from source reconstructed MEG data using data-driven analyses of neuronal synchronization. Oscillation amplitudes and interareal phase-synchronization in the 3–120 Hz frequency range were estimated for 400 cortical parcels and correlated with clinical symptoms and neuropsychological scores. ScZ patients were characterized by a reduction in γ-band (30–120 Hz) oscillation amplitudes that was accompanied by a pronounced deficit in large-scale synchronization at γ-band frequencies. Synchronization was reduced within visual regions as well as between visual and frontal cortex and the reduction of synchronization correlated with elevated clinical disorganization. Accordingly, these data highlight that ScZ is associated with a profound disruption of transient synchronization, providing critical support for the notion that core aspect of the pathophysiology arises from an impairment in coordination of distributed neural activity.
Myocardial injury as induced by myocardial infarction results in tissue ischemia, which critically incepts cardiomyocyte death. Endothelial cells play a crucial role in restoring oxygen and nutrient supply to the heart. Latest advances in single-cell multi-omics, together with genetic lineage tracing, reveal a transcriptional and phenotypical adaptation to the injured microenvironment, which includes alterations in metabolic, mesenchymal, hematopoietic and pro-inflammatory signatures. The extent of transition in mesenchymal or hematopoietic cell lineages is still debated, but it is clear that several of the adaptive phenotypical changes are transient and endothelial cells revert back to a naïve cell state after resolution of injury responses. This resilience of endothelial cells to acute stress responses is important for preventing chronic dysfunction. Here, we summarize how endothelial cells adjust to injury and how this dynamic response contributes to repair and regeneration. We will highlight intrinsic and microenvironmental factors that contribute to endothelial cell resilience and may be targetable to maintain a functionally active, healthy microcirculation.
Event-related potential (ERP) data in monolingual German speakers have shown that sentential metric expectancy violations elicit a biphasic ERP pattern consisting of an anterior negativity and a posterior positivity (P600). This pattern is comparable to that elicited by syntactic violations. However, proficient French late learners of German do not detect violations of metric expectancy in German. They also show qualitatively and quantitatively different ERP responses to metric and syntactic violations. We followed up the questions whether (1) latter evidence results from a potential pitch cue insensitivity in speech segmentation in French speakers, or (2) if the result is founded in rhythmic language differences. Therefore, we tested Spanish late learners of German, as Spanish, contrary to French, uses pitch as a segmentation cue even though the basic segmentation unit is the same in French and Spanish (i.e., the syllable). We report ERP responses showing that Spanish L2 learners are sensitive to syntactic as well as metric violations in German sentences independent of attention to task in a P600 response. Overall, the behavioral performance resembles that of German native speakers. The current data suggest that Spanish L2 learners are able to extract metric units (trochee) in their L2 (German) even though their basic segmentation unit in Spanish is the syllable. In addition Spanish in contrast to French L2 learners of German are sensitive to syntactic violations indicating a tight link between syntactic and metric competence. This finding emphasizes the relevant role of metric cues not only in L2 prosodic but also in syntactic processing.

Die Septische Granulomatose (CGD) ist eine seltene Erkrankung, die auf einem genetischen Defekt bestimmter weißer Blutzellen beruht, die darauf spezialisiert sind, in den Körper eingedrungene Pilze und Bakterien aufzuspüren und zu vernichten. Frankfurter Ärzten und Wissenschaftlern um Prof. Dr. Dieter Hoelzer vom Klinikum der Johann Wolfgang Goethe-Universität und Dr. Manuel Grez vom Chemotherapeutischen Forschungsinstitut Georg-Speyer-Haus gelang es, eine intakte Kopie des defekten Gens in Blutstammzellen von zwei erwachsenen CGD-Patienten einzuschleusen und so die Funktion der Fresszellen teilweise wieder herzustellen. Eine vollständige Heilung gelang jedoch nicht – ein Patient verstarb zwei Jahre nach der zunächst erfolgreichen Behandlung an seiner Grunderkrankung. Im Gespräch mit Dr. Anne Hardy berichten Dr. Marion Gabriele Ott (Arbeitsgruppe Hoelzer) und Dr. Manuel Grez (Georg-Speyer-Haus) über die Höhen und Tiefen ihrer gentherapeutischen Forschung.
Several studies suggested that transcription factor (TF) binding to DNA may be impaired or enhanced by DNA methylation. We present MeDeMo, a toolbox for TF motif analysis that combines information about DNA methylation with models capturing intra-motif dependencies. In a large-scale study using ChIP-seq data for 335 TFs, we identify novel TFs that are affected by DNA methylation. Overall, we find that CpG methylation decreases the likelihood of binding for the majority of TFs. For a considerable subset of TFs, we show that intra-motif dependencies are pivotal for accurately modelling the impact of DNA methylation on TF binding.
Bei seiner Einschulung hatte Wolfgang Niedecken ein besonderes Erlebnis, als er im Alter von 6 Jahren bemerkte, dass es höchste Zeit war, ordentlich sprechen zu lernen. Im Elternhaus war ausschließlich Kölsch gesprochen worden. Hochdeutsch war seine erste Fremdsprache, die sie in letzter Konsequenz auch immer geblieben ist. Denken, empfinden und träumen tut Niedecken nach wie vor auf Kölsch, wie er selbst beteuert.
Im Sommer 1976 schreibt Wolfgang Niedecken seinen ersten Song auf Kölsch: "Helfe kann dir keiner". 1979 erscheint sein erstes Album mit dem Titel: "Wolfgang Niedecken’s BAP rockt andere kölsche Leeder".
Nie hat sich BAP auf seinen Tourneen vor den „Karren politischer Machthaber“ spannen lassen. Zu Zeiten zweier deutscher Staaten sorgten die offenen Worte von „Deshalv spill`mer he“ für einen Eklat. Das Lied sprach sich unmissverständlich nicht nur für die westdeutsche Friedensbewegung, sondern ebenfalls für die ostdeutschen Friedens- und Menschenrechtsinitiativen aus. Als die DDR-Kulturbehörden BAP verboten, den Song zu spielen, platzt am Vorabend des ersten Konzertes im Berliner Palast der Republik die über 14 Stationen geplante und längst ausverkaufte DDR-Tournee.
Am 9. November 1992 findet das „Arsch huh“-Konzert gegen Ausländerfeindlichkeit und Rassismus auf dem Kölner Chlodwigplatz vor über 100.000 Menschen statt. Danach ähnliche Konzerte in Frankfurt „Heute die, morgen Du“ und in Leipzig „Gewalt ätzt“ vor ähnlich großer Kulisse. Für sein gesellschaftspolitisches Engagement bekommt Niedecken 1998 das Bundesverdienstkreuz von Bundespräsident Roman Herzog überreicht. In der Laudatio heißt es: „Kölsch-Rock, BAP und Kölner Dialekt sind untrennbar mit ihm verbunden. Er ist einer der profiliertesten Rockmusiker Deutschlands. Als engagierter Künstler hat er sich nachhaltig für Frieden, Toleranz, Demokratie und gegen Fremdenfeindlichkeit eingesetzt.“
Zu Niedeckens 60. Geburtstag, den er am 30. März 2011 mit etwa 500 Gästen auf einem Rheinschiff feiert, sendet der WDR die „Niedecken-Nacht“. Zu seinem Geburtstag erscheint im Verlag Hoffmann und Campe auch das Buch „Für 'ne Moment. Autobiographie“. Auf über 500 Seiten erzählt der Musiker von seiner Familie, einer behüteten frühen Kindheit und der schwierigen Zeit als Heranwachsender im katholischen Internat, von seinem Kunststudium und Aufenthalten in der New Yorker Kunstszene, der ehe zufälligen Gründung von BAP, die zur erfolgreichsten Mundartgruppe Deutschlands wurde.
Und dann das: Am 2. November gegen 13 Uhr bemerkt Wolfgang Niedecken beim Lesen, „dass ich nichts mehr kapierte. Ich musste die Seiten immer wieder neu lesen. Dann wurde es nebelig vor den Augen, alles sah merkwürdig aus, mein ganzes Umfeld hatte amorphe Formen. Und dann begegnete ich Gott sei Dank meinem Schutzengel.“ Wolfgang Niedecken hatte einen Schlaganfall erlitten, obwohl er eigentlich kein typischer Schlaganfallpatient war. Es kann also jeden treffen! Dank des schnellen Reagierens seiner Frau Tina konnte das Schlimmste verhindert werden.
Zielsetzung: Beteiligung von Medizinstudierenden im Rahmen der konzeptionellen Entwicklung eines zielgruppenspezifischen und attraktiven allgemeinmedizinischen Lehrangebots im ländlichen Raum.
Methodik: Es wurde ein Fragebogen entwickelt, der die Bewertung der Studierenden hinsichtlich des aktuellen Ablaufs ihres Studiums, den späteren Berufswunsch sowie die Anforderungen an ein zu entwickelndes allgemeinmedizinisches Schwerpunktprogramm im ländlichen Raum erfasst. Mittels einer Online-Befragung wurden im Sommer 2015 alle Medizinstudierende ab dem vierten vorklinischen Semester (n=2.150) der Goethe-Universität Frankfurt einmalig befragt. Die statistische Auswertung erfolgte primär deskriptiv. Die persönliche Einstellung hinsichtlich der Bereitschaft, als Hausarzt tätig zu werden, wurde auf statistische Signifikanz überprüft. Zudem wurde erhoben, ob ein messbarer Zusammenhang zwischen der eigenen Herkunft und dem späteren Wunscharbeitsort besteht.
Ergebnisse: Von insgesamt 2.150 kontaktierten Studierenden nahmen 617 an der Befragung teil (Rücklaufquote=28,7%). Die Ergebnisse repräsentieren eine große Bandbreite an Ideen und Anregungen, die sowohl die Meinung von Befürwortern als auch eher kritisch gegenüber der Lehre in der Allgemeinmedizin eingestellten Medizinstudierenden widerspiegeln. Von dem geplanten Schwerpunktprogramm erwarten die Studierenden einen starken Praxisbezug ebenso wie das Kennenlernen administrativer sowie wirtschaftlicher Hintergründe zum Führen einer Praxis.
Schlussfolgerungen: Durch die Einbeziehung der Zielgruppe am Entwicklungsprozess bestand die Möglichkeit, das zu entwickelnde Schwerpunktprogramm auf die späteren Teilnehmer passgenauer zuzuschneiden. Zudem ist zu erwarten, dass die Beteiligung der Studierenden zu einer höheren Akzeptanz des Programms führt. Die gewonnenen Ergebnisse zur Gestaltung eines Lehrangebots können als Orientierung für die mögliche Entwicklung ähnlicher Schwerpunktprogramme an anderen medizinischen Fakultäten dienen.
Wirkungen von Heilpflanzen, Gewürzen, Tees und Lebensmitteln werden in der Naturheilkunde seit der Antike genutzt. Pharmakologisch wirksam sind in der Regel nur die sekundären Pflanzeninhaltsstoffe. Diese in den oft aus vielen Bestandteilen zusammengesetzten Naturstoffen aufzuspüren und ihren molekularbiologischen Wirkungsmechanismus im Körper aufzuklären, ist das Ziel eines Forschungsnetzwerks am Frankfurter ZAFES (Zentrum für Arzneimittelforschung, -Entwicklung und -Sicherheit). So konnten Pharmazeuten und Kliniker gemeinsam herausfinden, wie ein Bestandteil des Rotweins, das Resveratrol, vor Darmkrebs schützt. Die Inhaltsstoffe von Salbei und Rosmarin bieten vielversprechende Ausgangspunkte für neue Medikamente gegen Altersdiabetes. Weihrauch, Myrte und Johanniskraut enthalten Wirkstoffe, die Schlüsselenzyme für Entzündungsreaktionen – etwa bei rheumatischen Beschwerden – hemmen.

One of the crucial steps during trials for Zika and other vaccines is to recruit participants and to understand how participants’ attitudes and sociodemographic characteristics affect willingness to participate (WTP). This study was conducted to assess WTP, its explanatory variables, and the impact of financial compensation on WTP in Indonesia. A health facility-based cross-sectional study was conducted in eleven regencies in the Aceh and West Sumatra provinces of Indonesia. Participants were recruited via a convenience sampling method and were interviewed. The associations between explanatory variables and WTP were assessed using a two-step logistic regression analysis. A total of 1,102 parents were approached, and of these 956 (86.8%) completed the interview and were included in analysis. Of those, 144 (15.1%) were willing to participate in a Zika vaccine trial without a financial compensation. In the multivariate analysis, WTP was tied to an age of more than 50 years old, compared to 20–29 years (odds ratio (OR): 5.0; 95% confidence interval (CI): 2.37–10.53), to being female (OR: 2.20; 95% CI: 1.11–4.37), and to having heard about Zika (OR: 2.41; 95% CI: 1.59–3.65). Participants’ WTP increased gradually with higher financial compensation. The rate of WTP increased to 62.3% at the highest offer (US$ 350.4), and those who were still unwilling to participate (37.7%) had a poorer attitude towards childhood vaccination. This study highlights that pre-existing knowledge about Zika and attitudes towards childhood vaccination are important in determining community members being willing to participate in a vaccine trial. Financial incentives are still an important factor to enhance participant recruitment during a vaccine trial.
Background: Correct species identification of blow flies is a crucial step for understanding their biology, which can be used not only for designing fly control programs, but also to determine the minimum time since death. Identification techniques are usually based on morphological and molecular characters. However, the use of classical morphology requires experienced entomologists for correct identification; while molecular techniques rely on a sound laboratory expertise and remain ambiguous for certain taxa. Landmark-based geometric morphometric analysis of insect wings has been extensively applied in species identification. However, few wing morphometric analyses of blow fly species have been published.
Methods: We applied a landmark-based geometric morphometric analysis of wings for species identification of 12 medically and forensically important blow fly species of Thailand. Nineteen landmarks of each right wing of 372 specimens were digitised. Variation in wing size and wing shape was analysed and evaluated for allometric effects. The latter confirmed the influence of size on the shape differences between species and sexes. Wing shape variation among genera and species were analysed using canonical variates analysis followed by a cross-validation test.
Results: Wing size was not suitable for species discrimination, whereas wing shape can be a useful tool to separate taxa on both, genus and species level depending on the analysed taxa. It appeared to be highly reliable, especially for classifying Chrysomya species, but less robust for a species discrimination in the genera Lucilia and Hemipyrellia. Allometry did not affect species separation but had an impact on sexual shape dimorphism.
Conclusions: A landmark-based geometric morphometric analysis of wings is a useful additional method for species discrimination. It is a simple, reliable and inexpensive method, but it can be time-consuming locating the landmarks for a large scale study and requires non-damaged wings for analysis.
Die Kombination eines niedrig dosierten Lokalanästhetikums mit dem Opioid Sufentanil zur Patientinnen kontrollierten Epiduralanalgesie (PCEA) gewährleistet eine exzellente geburtshilfliche Schmerztherapie. Bislang konnte die minimale Sufentanilkonzentration in Kombination mit Ropivacain, die eine effektive Analgesie zur schmerzarmen Geburt in Form einer PCEA gewährleistet, nicht bestimmt werden. Verglichen wurden drei verschiedene Sufentanildosierungen im Bezug auf ihre analgetische Wirksamkeit zur schmerzarmen Geburt. Zusätzlich wurden erstmals die maternalen und neonatalen Plasmakonzentrationen für Sufentanil kombiniert verabreicht mit Ropivacain nach PCEA zur schmerzarmen Geburt bestimmt. Nach institutioneller Ethikkommissionsgenehmigung und schriftlicher Aufklärung sowie Einverständniserklärung wurden 74 Patientinnen, (ASA I und II, Einlingsschwangerschaft, Hinterhauptslage, 30 ± 6 Jahre, 77 ± 12 kg, 167 ± 7 cm, 39 ± 2 SSW) in diese prospektive, randomisierte Doppelblindstudie eingeschlossen. Die Patientinnen erhielten 0,16% Ropivacain in Kombination mit 0,25 (n = 29), 0,5 (n = 23) oder 0,75 Mikrogramm/mL (n = 22) Sufentanil. 30 Minuten nach Gabe der Volldosis, bestehend aus 16 mg Ropivacain plus 10 Mikrogramm Sufentanil, wurde die PCEA-Pumpe (Basalrate 4 mL/h, Bolus 3 mL, Lockout-Zeit: 15 min, maximal Dosis/h: 16mL) mit den drei unterschiedlichen Sufentanildosierungen gestartet. Bis zu sechs Stunden nach Beginn der PCEA wurde die Gesamtmenge an appliziertem Ropivacain und Sufentanil, die Schmerzintensität der Schwangeren (Visuelle Analog Skala, VAS 0-100 mm), der Anteil der Schwangeren mit breakthrough pain (VAS-Score > 40) und die mütterliche Zufriedenheit mit dem Verfahren bestimmt. Nach der Abnabelung des Neugeborenen wurde 5 mL maternales Blut aus der Venenverweilkanüle und 5 mL Blut aus der Nabelschnurarterie des Kindes entnommen. Das Plasma wurde durch Zentrifugation separiert und bis zur Analyse bei –40 °C asserviert. Die Sufentanilplasmaspiegelbestimmung erfolgte mit Hilfe einer Flüssigkeitschromatographie, kombiniert mit Elektrospray-Ionisierung-Tandem-Massenspektrometrie (LC-ESI-MS/MS) und deuteriertem Sufentanil, [2H5]-Sufentanil, als internem Standard. Die untere Nachweisgrenze der neuen Bestimmungsmethode liegt bei 0,25 pg/mL (12,5 fg on column) und ist damit deutlich empfindlicher als bei bisherigen Nachweisverfahren (Schmidt 2006, Martens-Lobenhoffer 2002 und Loftus 1995). Es gab zwischen den drei Gruppen keine Unterschiede in den demographischen Daten, bei der Geburtsdauer und der Gesamtmenge an verabreichten Ropivacain. Im Bezug auf die angeforderten und applizierten Boli pro Stunde gab es zwischen den drei Gruppen keine statistisch signifikanten Unterschiede. Zu keinem Zeitpunkt gab es einen statistisch signifikanten Unterschied bezüglich der Schmerzintensität der Schwangeren (VAS-Score), der Inzidenz an breakthrough pain und der mütterlichen Zufriedenheit. Dosisabhängig bestand zwischen den drei Gruppen ein statistisch signifikanter Unterschied sowohl im Bezug auf die applizierte Sufentanilmenge (p < 0,001), als auch bei den maternalen und neonatalen Sufentanilplasmakonzentrationen (p < 0,001) zum Zeitpunkt der Geburt. Für die Gruppe mit der niedrigsten Sufentanilkonzentration ließ sich eine niedrigere Sectiorate als erwartet nachweisen (s. Tab. 15). .... Tabelle 15: Angaben in der jeweiligen Einheit, als Median und Inter-Quartil-range. PCEA = Patientinnen kontrollierte epidurale Analgesie, min = Minuten und VAS = Visuelle Analog-Skala. Die drei Sufentanildosierungen (0,25, 0,5 und 0,75 Mikrogramm/mL) bewirken eine vergleichbar gute Analgesie und exzellente Patientinnenzufriedenheit. Auf Grund dieser Untersuchungsergebnisse kann die Kombination von 0,25 Mikrogramm/mL Sufentanil mit Ropivacain 0,16%, als PCEA zur schmerzarmen Geburt verabreicht, empfohlen werden. Auch bei prolongiertem Geburtsverlauf (> 6 h PCEA) kann das Dosislimit in der Produktinformation von 30 Mikrogramm Sufentanil sicher eingehalten werden. Die maternalen und neonatalen Sufentanilplasmakonzentrationen liegen, selbst bei längerem Geburtsverlauf mit überschreiten des oberen Dosislimits, im niedrigen Picogrammbereich und damit niedriger als bei bisher publizierten Daten (Brodner 2000, Loftus 1995 und Cohen 1996). Ob die epidural verabreichte Sufentanildosierung einen Einfluss auf den Entbindungsmodus ausübt, können nur Studien mit variabler Opioidkonzentration und deutlich höherer Fallzahl klären.
Wirksamkeit von Articain und Lidocain bei verschiedenen Verfahren der zahnärztlichen Lokalanästhesie

Articain und Lidocain sind die in der Zahnheilkunde am häufigsten verwendete Lokalanästhetika. Beide Substanzen besitzen eine ähnliche lokalanästhetische Wirksamkeit, sind jedoch in unterschiedlich konzentrierter Lösung im Handel. Articain ist das einzige Lokalanästhetikum, das einerseits in hoher Konzentration (4 %), andererseits mit gering konzentriertem Adrenalin-Zusatz (1:200.000 und 1:400.000) verfügbar ist. Lokalanästhetika werden nach der Wirkung dosiert. Wenn die lokalanästhetische Wirkung aufgrund zu geringer Konzentration von Lokalanästhetikum oder Vasokonstriktor unzureichend ist, kann durch eine Nachinjektion in der Regel eine vollständige Wirkung erzielt werden. Da bei der intraligamentären Injektionstechnik nur sehr geringe Volumina in den engen Desmodontalspalt injiziert werden können, kann bei dieser Technik eine unzureichende Wirkung nicht durch eine Dosiserhöhung kompensiert werden. Daher erscheint für die intraligamentäre Anästhesie eine höher konzentrierte Lösung als geeignet. In einer Vergleichsuntersuchung sollte die lokalanästhetische Wirkung handelsüblicher Articain- und Lidocain-Lösungen für die intraligamentäre Anästhesie überprfüt werden. In einer 6-fach-cross-over-Untersuchung erfolgte bei 25 gesunden Probanden an einem Prämolaren eine Lokalanästhesie durch submuköse Infiltration und intraligamentäre Injektion mit 4 % Articain mit Adrenalin 1:200.000 bzw. 1:100.000 sowie 2 % Lidocain mit Adrenalin 1:100.000. Die Anästhesietiefe wurde mittels elektrischer Sensibilitätsprüfung bestimmt. Eine vollständige Anästhesie konnte bei Infiltrationsanästhesie in praktisch allen Fällen erzielt werden, die Anästhesiedauer zeigte zwischen den drei lokalanästhetischen Lösungen keine Unterschiede. Bei der intraligamentären Anästhesie zeigten sich deutliche Unterschiede zwischen den 4-prozentigen und der 2-prozentigen Lösung. Sowohl der Anteil der vollständigen Anästhesiewirkung als auch die Dauer der Anästhesie war bei den beiden 4-prozentigen Lösungen deutlich höher als bei der 2-prozentigen Lösung. Aufgrund der Ergebnisse der vorliegenden Untersuchung sind 4- prozentige Articain-Lösungen für die intraligamentäre Anästhesie uneingeschränkt geeignet, wobei die Zubereitung mit der höheren Adrenalin-Konzentration keine Vorteile bietet. Die handelsübliche 2-prozentige Lidocain-Lösung ist aufgrund deutlich geringerer Wirksamkeit für diese Technik weniger geeignet.
Hintergrund: Es ist unbekannt bzw. umstritten, ob die in Deutschland eingeführten Umweltzonen (UWZ) die Feinstaubbelastung nachweisbar reduzieren.
Methode: PM10-Konzentrationen von den Messstationen innerhalb und außerhalb der UWZ in 19 deutschen Städten wurden analysiert (Augsburg, Berlin, Dortmund, Duisburg, Düsseldorf, Essen, Frankfurt a. M., Hannover, Herrenberg, Ilsfeld, Karlsruhe, Köln, Ludwigsburg, Mannheim, München, Reutlingen, Stuttgart, Tübingen, Wuppertal), um die Wirksamkeit der Fahrverbote (Stufe 1) für Fahrzeuge der Schadstoffgruppe 1 (ohne Plakette) auf die Schadstoffkonzentration zu untersuchen. Kontinuierliche Halbstundenmesswerte und gravimetrische Tagesmittelwerte wurden für den Zeitraum von ca. 2005 bis Ende 2009 übernommen. Die Analyse beruht auf vier einander paarweise zugeordneten Messwerten als gematchte Quadrupel aus zwei Index- und zwei Referenzwerten (Indexstationen liegen innerhalb, Referenzstationen messen außerhalb der UWZ). Ein Indexwert und der simultan gemessene Referenzwert wurden während der aktiven Phase der UWZ gemessen, das andere Wertepaar wurde vor Einführung der UWZ erhoben. Die Wertepaare haben eine Zeitdifferenz von 364 Tagen oder von einem Vielfachen von 364 Tagen, wodurch die Jahreszeit, der Wochentag und die Tageszeit im Quadrupel konstant gehalten werden. Differenzen der Indexwerte wurden regressionstechnisch mit den Differenzen der Referenzwerte korrigiert, wobei meteorologische Parameter (Mischungsschichthöhe, Niederschlagsmenge, Windgeschwindigkeit), Schulferienzeiten, Phase der Umweltprämie, LKW-Fahrverbotszeiten und Ausgangswerte an den Index- und Referenzstationen als Kovariablen in sog. „fixed effects“ Regressionsanalysen der Quadrupel berücksichtigt wurden (Differenzwertmethode im Zwei-Perioden-Fall). Dieser statistische Ansatz wurde vor der eigentlichen Datenanalyse an simulierten Messdaten der FU Berlin erfolgreich erprobt.
Ergebnisse: 2 110 803 Quadrupel kontinuierlicher PM10-Messungen und 15 735 gravimetrische Quadrupel wurden aus den verfügbaren Daten der Messstationen identifiziert, aus denen 61 169 Quadrupel zu Tagesmittelwerten aufgebaut wurden. Die Analysen für die erste Stufe ergaben als beste Effektschätzer (an allen Indexstationen) eine Feinstaubreduktion von ≤ 0,2 μg/m3 (bzw. relative PM10-Reduktionen ≤ 1 %). Der beste Effektschätzer an allen Verkehrsstationen (also ohne städtische Hintergrund- und Industrieindexstationen) lag unterhalb von 1 μg/m3 (bzw. weniger als 5 %).
Schlussfolgerungen: Alle Analysewerte liegen damit unter den vor Einführung von UWZ prognostizierten Feinstaubreduktionen. Diese Studie untersuchte als erste übergreifend die Wirksamkeit von UWZ der Stufe 1 in Deutschland auf die Feinstaubkonzentrationen von PM10 nach einem einheitlichen Datensammlungs- und Analyseplan und unter Berücksichtigung möglichst vieler Störeinflüsse.
Einleitung: Beim allergischen Asthma bronchiale handelt es sich um eine weltweit zunehmende Erkrankung, für die es bislang nur wenige kausale Therapien gibt. In der Therapie chronisch-entzündlicher Erkrankungen wie der chronischen Rhinitis, Sinusitis, Akne vulgaris oder auch dem Asthma bronchiale, werden seit vielen Jahrzehnten autologe Autovaccine eingesetzt und stellen eine effektive Behandlungsoption dar.
Methoden: In einer prospektiven, randomisierten, doppelblinden Studie wurden 31 Probanden mit einem Asthma GINA I° sowie einem positiven Prick-Hauttest und einer positiven bronchialen Allergenprovokation mit Hausstaubmilbe entweder mit einer autologen E. coli Autovaccine (AV) oder einem Placebo über 24 Wochen behandelt, woraufhin eine abschließende Provokationsphase folgte. Als primären Zielparameter bestimmten wir das exhalative Stickstoffmonoxid (eNO) als Marker der bronchialen Entzündung. Die sekundären Zielparameter waren die bronchiale Hyperreagibilität vor und nach Behandlung sowie nach Provokation, das Allergie-Labor (Gesamt-IgE, spezifisches IgE gegen Hausstaubmilbe), die klinische Verträglichkeit sowie die Spätreaktion und der Medikamentenverbrauch in der abschließenden Provokationswoche.
Ergebnisse: Die Patienten der AV-Gruppe (36,7 ppb) fielen unter Behandlung signifikant auf 24,2 ppb ab, während die Placebo-Gruppe (26,8 ppb) mit 30,6 ppb eher angestiegen war. Bezüglich der bronchialen Hyperreagibilität ergab sich in der AV-Gruppe ebenfalls eine deutliche Verbesserung im Vergleich zur Placebo-Gruppe: Placebo 1,17 mg vs. AV 0,51 mg vor Behandlung auf Placebo 1,03 mg vs. AV 0,99 mg nach Behandlung. Sowohl bezüglich der BHR als auch der eNO-Werte konnten diese Ergebnisse unter Provokation nicht bestätigt werden. In beiden Gruppen stieg das eNO unter den abschließenden bronchialen Allergenprovokationen signifikant auf 104,4 ppb in der Placebo-Gruppe und 91,1 ppb in der AV-Gruppe an. Während der abschließenden Provokationsphase zeigten sich in der AV-Gruppe ein signifikant geringerer Salbutamol-Bedarf sowie ein signifikant geringeres Auftreten von Spätreaktionen (LAR) im Vergleich zur Placebo-Gruppe. Es traten nur leichte, selbst limitierende Lokalreaktionen auf.
Diskussion: Die Anwendung einer autologen E. coli Autovaccine war sicher und gut verträglich. Die Ergebnisse zeigten einen positiven Einfluss auf die bronchiale Entzündung und Hyperreagibilität bei leichten Asthmatikern mit Hausstaubmilben-Allergie und stellen so eine neue Behandlungsoption dar. Zum genauen Wirkmechanismus der AV bedarf es weiterer Studien.
Wirkung von Glia-Maturationsfaktor (GMF) und Adhäsionsfaktor (ADF) auf Kulturen leukämischer Zellen

GMF was isolated from the glia of different brain parts of calf. Its influence on the input of 3H-TdR was tested in leukaemic and non-malignant lymphocites. GMF isolated from the diencephalon proved to be the most effective one. The used substance of reference was DBcAMP. The cAMP contents of leukaemic cells were lower than those of non-malignant cells, whereas the level of cGMP was higher in the first ones. We found both in non-malignant cultures and in cultures of leukaemic cells that the impact of GMF and AdF elevated the cAMP value and reduced cGMP. The adhesion of cAMP to the nuclei of leukaemic was reduced in comparison with non-malignant cells. The nuclei of leukaemic cells showed increased adhesive capacity after pre-incubation with GMF and AdF. The adhesive capacity of non-malignant nuclei remained unchanged. The adenylcyclase activity (AC) was diminished in leukaemic cells. It could not be stimulated by means of catecholamines. GMF and AdF produced a slight increase of the basal Ac activity in cultures of leukaemic cells only. But isoproterenol led to a distinct increase of the Ac activity in leukaemic cells when incubation with those two factors preceded it.
Bei der vorliegenden prospektiven und verblindeten klinischen Vergleichsstudie wurde die Wirkung prophylaktisch inhalierten Iloprosts bei 40 Patienten ohne bekannte pulmonale Hypertonie untersucht, die sich einer elektiven chirurgischen Revaskularisierung der Koronargefäße unter Verwendung einer extrakorporalen Zirkulation (EKZ) unterzogen. Bei derartigen Operationen kommt es regelhaft zu einer generalisierten Entzündungsreaktion gefolgt von einer Rechtsherzbelastung unterschiedlichen Ausmaßes, was den postoperativen Verlauf teilweise erheblich kompliziert. Zur Therapie einer in diesem Rahmen aufgetretenen Rechtsherzbelastung hat sich inhaliertes Iloprost bereits vielfach als sehr wirksam erwiesen. Zusätzlich ist seit einiger Zeit auch eine antiinflammatorische Wirkung von Iloprost bekannt. Zielsetzung dieser Studie war es daher herauszufinden, ob auch die prophylaktische Applikation eine vorteilhafte Wirkung auf perioperative Hämodynamik und Oxygenierung sowie auf perioperative Inflammationsreaktion zeigen kann. Dazu wurde vor EKZ 20 Patienten 10µg aerosolisiertes Iloprost per inhalationem appliziert, 20 Patienten der Kontrollgruppe inhalierten 0,9%ige NaCl–Lösung. Nach Ende der EKZ wurden hämodynamische Parameter mittels Pulskonturanalyse (PiCCO®) und dessen Rechtsherz–Erweiterungsmonitors (VoLEF®) protokolliert sowie arterielle und gemischtvenöse Blutgasanalysen durchgeführt. Weiterhin wurden pro– und antiinflammatorische Zytokine (Interleukin–1beta, 6, 8 und 10 sowie Tumor–Nekrose–Faktor alpha) in Blutplasma und bronchoalveolärer Lavageflüssigkeit bestimmt Die Ergebnisse bei den meisten beobachteten Parametern in Studien– und Kontrollguppe unterschieden sich nicht signifikant voneinander. Jedoch zeigte sich in der Studiengruppe ein erhöhtes HZV sowie ein höherer kardialer Funktionsindex als Marker der Inotropie bei gleichzeitig niedrigerem systemischem Gefäßwiderstand. Da diese Wirkung nach Ende der üblichen Wirkdauer von Iloprost auftrat, interpretieren wir dies als einen Hinweis auf eine verbesserte hämodynamische Stabilität durch die prophylaktische Inhalation von Iloprost. Ein weiteres Ergebnis war ein etwas besseres Sauerstoffangebot in der Studiengruppe gegen Ende der Untersuchung. Die Analyse der Zytokinbestimmungen aus Blutplasma und Bronchialsekret konnte keine signifikanten Unterschiede zwischen den beiden Gruppen zeigen, eine antiinflammatorische Wirkung inhalierten Iloprosts konnte hier nicht dargestellt werden. Auch eine relevante Rechtsherzbelastung wurde nicht beobachtet. Da inhaliertes Iloprost seine vorteilhafte Wirkung auf postoperative Hämodynamik und kardiale Pumpleistung vor allem durch Senkung pathologisch erhöhter intrapulmonaler Widerstände entfaltet und eine antiinflammatorische Wirkung erst bei einer Inflammation zum Tragen kommen kann, ist zu erwägen ob nicht der inflammatorische Reiz bei dem untersuchten verhältnismäßig gesunden Patientenkollektiv zu gering war, um die vermuteten Effekte zu zeigen. Im Verlauf der Studie ist kein schwerer Zwischenfall aufgetreten, auch konnte kein nachteiliger Effekt prophylaktisch inhalierten Iloprosts bei dem vorliegenden Patientenkollektiv nachgewiesen werden. Eine Studie, die die Auswirkung von inhaliertem Iloprost auf Hämodynamik, Oxygenierung und Inflammation an kardial vorbelasteten Patienten mit langer Operations– und EKZ–Dauer untersucht, erscheint daher sinnvoll.
Neues Wissen erzeugt gleichzeitig auch Nichtwissen. Wie damit umzugehen ist, wirft in kaum einem Forschungsgebiet so viele Fragen auf wie in der Medizin. So kann die Pränataldiagnostik heute schon im ersten Schwangerschaftsdrittel bestimmte, früher nicht behandelbare Fehlbildungen erkennen. Dadurch entstehen komplexe Behandlungssituationen, die Eltern mit bisher nicht bekannten Unwägbarkeiten konfrontieren. Forschende aus Medizin und Soziologie untersuchen gemeinsam, wie Eltern während und nach der Schwangerschaft auf die schwierige Situation reagieren.
Background: Using data from the COHERE collaboration, we investigated whether primary prophylaxis for pneumocystis pneumonia (PcP) might be withheld in all patients on antiretroviral therapy (ART) with suppressed plasma human immunodeficiency virus (HIV) RNA (≤400 copies/mL), irrespective of CD4 count.
Methods: We implemented an established causal inference approach whereby observational data are used to emulate a randomized trial. Patients taking PcP prophylaxis were eligible for the emulated trial if their CD4 count was ≤200 cells/µL in line with existing recommendations. We compared the following 2 strategies for stopping prophylaxis: (1) when CD4 count was >200 cells/µL for >3 months or (2) when the patient was virologically suppressed (2 consecutive HIV RNA ≤400 copies/mL). Patients were artificially censored if they did not comply with these stopping rules. We estimated the risk of primary PcP in patients on ART, using the hazard ratio (HR) to compare the stopping strategies by fitting a pooled logistic model, including inverse probability weights to adjust for the selection bias introduced by the artificial censoring.
Results: A total of 4813 patients (10 324 person-years) complied with eligibility conditions for the emulated trial. With primary PcP diagnosis as an endpoint, the adjusted HR (aHR) indicated a slightly lower, but not statistically significant, different risk for the strategy based on viral suppression alone compared with the existing guidelines (aHR, .8; 95% confidence interval, .6–1.1; P = .2).
Conclusions: This study suggests that primary PcP prophylaxis might be safely withheld in confirmed virologically suppressed patients on ART, regardless of their CD4 count.
Reproducing the characteristics and the functional responses of the blood–brain barrier (BBB) in vitro represents an important task for the research community, and would be a critical biotechnological breakthrough. Pharmaceutical and biotechnology industries provide strong demand for inexpensive and easy-to-handle in vitro BBB models to screen novel drug candidates. Recently, it was shown that canonical Wnt signaling is responsible for the induction of the BBB properties in the neonatal brain microvasculature in vivo. In the present study, following on from earlier observations, we have developed a novel model of the BBB in vitro that may be suitable for large scale screening assays. This model is based on immortalized endothelial cell lines derived from murine and human brain, with no need for co-culture with astrocytes. To maintain the BBB endothelial cell properties, the cell lines are cultured in the presence of Wnt3a or drugs that stabilize β-catenin, or they are infected with a transcriptionally active form of β-catenin. Upon these treatments, the cell lines maintain expression of BBB-specific markers, which results in elevated transendothelial electrical resistance and reduced cell permeability. Importantly, these properties are retained for several passages in culture, and they can be reproduced and maintained in different laboratories over time. We conclude that the brain-derived endothelial cell lines that we have investigated gain their specialized characteristics upon activation of the canonical Wnt pathway. This model may be thus suitable to test the BBB permeability to chemicals or large molecular weight proteins, transmigration of inflammatory cells, treatments with cytokines, and genetic manipulation.
The blood–brain barrier (BBB) is confined to the endothelium of brain capillaries and is indispensable for fluid homeostasis and neuronal function. In this study, we show that endothelial Wnt/beta-catenin (beta-cat) signaling regulates induction and maintenance of BBB characteristics during embryonic and postnatal development. Endothelial specific stabilization of beta-cat in vivo enhances barrier maturation, whereas inactivation of beta-cat causes significant down-regulation of claudin3 (Cldn3), up-regulation of plamalemma vesicle-associated protein, and BBB breakdown. Stabilization of beta-cat in primary brain endothelial cells (ECs) in vitro by N-terminal truncation or Wnt3a treatment increases Cldn3 expression, BBB-type tight junction formation, and a BBB characteristic gene signature. Loss of beta-cat or inhibition of its signaling abrogates this effect. Furthermore, stabilization of beta-cat also increased Cldn3 and barrier properties in nonbrain-derived ECs. These findings may open new therapeutic avenues to modulate endothelial barrier function and to limit the devastating effects of BBB breakdown.
Background: Wnt signaling controls the balance between stem cell proliferation and differentiation and body patterning throughout development. Previous data demonstrated that non-canonical Wnts (Wnt5a, Wnt11) increased cardiac gene expression of circulating endothelial progenitor cells (EPC) and bone marrow-derived stem cells cultured in vitro. Since previous studies suggested a contribution of the protein kinase C (PKC) family to the Wnt5a-induced signalling, we investigated which PKC isoforms are activated by non-canonical Wnt5a in human EPC. Methodology/Principal Findings: Immunoblot experiments demonstrated that Wnt5a selectively activated the novel PKC isoform, PKC delta, as evidenced by phosphorylation and translocation. In contrast, the classical Ca2+-dependent PKC isoforms, PKC alpha and beta2, and one of the other novel PKC isoforms, PKC epsilon, were not activated by Wnt5a. The PKC delta inhibitor rottlerin significantly blocked co-culture-induced cardiac differentiation in vitro, whereas inhibitors directed against the classical Ca2+-dependent PKC isoforms or a PKC epsilon-inhibitory peptide did not block cardiac differentiation. In accordance, EPC derived from PKC delta heterozygous mice exhibited a significant reduction of Wnt5a-induced cardiac gene expression compared to wild type mice derived EPC. Conclusions/Significance: These data indicate that Wnt5a enhances cardiac gene expressions of EPC via an activation of PKC delta.

Background: The aim of this study was to investigate the acceptance and assessment of work shadowing carried out by students and dentists in dental practices. Furthermore, the extent to which students perceive an improvement in their specialised, communication and social competencies, was to be examined.
Methods: 61 dental students in their clinical semesters at a German university participated in work shadowing placements at 27 different general dental practices. Before beginning, they received checklists of various competencies that they self-assessed using school grades (from 1 = "very good", to 6 = "failed"), which they also repeated after completion. The dentists supplemented this with their external assessments. In addition, the students were requested to fill out a 54-item questionnaire and compose a freely-structured report after the work shadowing; the dentists filled out a questionnaire containing 16 items. The statistical analysis was carried out by means of the Friedman Test, including a post-hoc test (Bonferroni-Holm correction).
Results: The analysis showed a significant overall improvement in the students’ self-assessed competencies by 0.71* ± 0.43 grades. With an average of 0.33* ± 0.36, the dentists’ external assessment proved significantly higher than the self-assessment. The greatest improvements were perceived by the students in the areas of accounting (1.17* ± 0.77), practice organisation (1.05* ± 0.61) and dentist’s discussions (0.94* ±0.80) [*p < 0.05]. The students confirmed experiencing an expansion of knowledge, an improvement in their communication skills and indicated a high degree of satisfaction in regard to the dentists (school grade 1.58 ± 0.93). A maximum amount of satisfaction towards the work shadow students was demonstrated by the dentists, and this form of teaching was assessed with a school grade of 1.69 ± 0.89.
Conclusion: Both students and dental practitioners demonstrated a high level of satisfaction in regard to the work shadowing. The students felt their knowledge had increased, viewed the dentists as motivating role models and acknowledged a significant improvement in their specialised, communication and social competencies. Work shadowing in dental teaching practices presents a sensible addition to academic teaching at a university.
Background: Only few studies deal with the workload of physical therapists and the health consequences, although this occupational group is quite important for the health care system in many industrialized countries (e.g. ca. 136 000 people are currently employed as physical therapists in Germany). Therefore, the current state of knowledge of work-related diseases and disorders of physical therapists is insufficient. The aim of the "Physical Therapist Cohort" (PTC) study is to analyze the association between work-related exposures and diseases among physical therapists in Germany. This article describes the protocol of the baseline assessment of the PTC study.
Methods/Design: A cross-sectional study will be conducted as baseline assessment and will include a representative random sample of approximately 300 physical therapists employed in Germany (exposure group), and a population-based comparison group (n = 300). The comparison group will comprise a sample of working aged (18–65 years) inhabitants of a German city. Variables of interest will be assessed using a questionnaire manual including questions regarding musculoskeletal, dermal, and infectious diseases and disorders as well as psychosocial exposures, diseases and disorders. In addition to subjective measures, a clinical examination will be used to objectify the questionnaire-based results (n = 50).
Discussion: The study, which includes extensive data collection, provides a unique opportunity to study the prospective association of work-related exposures and associated complaints of physical therapists. Baseline results will give first clues with regard to whether and how prevalent main exposures of physiotherapeutic work and typical work areas of physical therapists are associated with the development of work-related diseases. Thereby, this baseline assessment provides the basis for further investigations to examine causal relationships in accordance with a longitudinal design.
Background: Depression and anxiety are the most prevalent mental health difficulties in the workplace, costing the global economy $1 trillion each year. Evidence indicates that symptoms may be reduced by interventions in the workplace. This paper is the first to systematically review psychosocial interventions for depression, anxiety, and suicidal ideation and behaviours in small-to medium-size enterprises (SMEs).
Methods: A systematic search following PRISMA guidelines, registered in PROSPERO (CRD42020156275), was conducted for psychosocial interventions targeting depression, anxiety, and suicidal ideation/behaviour in SMEs. The PubMed, PsycINFO, Scopus, and two specific occupational health databases were searched, as well as four databases for grey literature, without time limit until 2nd December 2019.
Results: In total, 1283 records were identified, 70 were retained for full-text screening, and seven met the inclusion criteria: three randomised controlled trials (RCTs), three before and after designs and one non-randomised trial, comprising 5111 participants. Study quality was low to moderate according to the Quality Assessment Tool for Quantitative Studies. Five studies showed a reduction in depression and anxiety symptoms using techniques based on cognitive behavioural therapy (CBT), two reported no significant change.
Limitations: Low number and high heterogeneity of interventions and outcomes, high attrition and lack of rigorous RCTs.
Conclusions: Preliminary evidence indicates CBT-based interventions can be effective in targeting symptoms of depression and anxiety in SME employees. There may be unique challenges to implementing programmes in SMEs. Further research is needed in this important area.
Background: Despite the numerous associations of vitamin D with health and disease, vitamin D deficiency is still common from a global perspective. While basic research, clinical and preventive activities grow constantly in vitamin D research, there is no in-depth analysis of the related global scientific productivity available so far.
Methods: Density equalizing mapping procedures (DEMP) were combined with socioeconomic benchmarks using the NewQIS platform.
Results: A total of 25,992 vitamin D-related research articles were identified between 1900 to 2014 with a significant increase (r2 = .6541) from 1900 to 2014. Authors located in Northern America – especially in the USA – distributed the majority of global vitamin D research, followed by their Western European counterparts. DEMP-analysis illustrates that Africa and South America exhibit only minor scientific productivity. Among high-income group countries, Scandinavian nations such as Denmark or Finland (2147.9 and 1607.7 vitamin D articles per GDP in 1000 billion USD) were highly active with regard to socioeconomic figures.
Conclusion: Networks dedicated to vitamin D research are present around the world. Overall, the Northern American and Western European nations occupy prominent positions. However, South American, African and Asian countries apart from Japan only play a minor role in the global research production related to vitamin D. Since vitamin D deficiency is currently increasing in the Americas, Europe and parts of the Middle East, research in these regions may need to be encouraged.
Background: A large number of idiosyncratic drug induced liver injury (iDILI) and herb induced liver injury(HILI) cases of variable quality has been published but some are a matter of concern if the cases were not evaluated for causality using a robust causality assessment method (CAM) such as RUCAM (Roussel Uclaf Causality Assessment Method) as diagnostiinjuryc algorithm. The purpose of this analysis was to evaluate the worldwide use of RUCAM in iDILI and HILI cases. Methods: The PubMed database (1993–30 June 2020) was searched for articles by using the following key terms: Roussel Uclaf Causality Assessment Method; RUCAM; Idiosyncratic drug induced liver injury; iDILI; Herb induced liver injury; HILI. Results: Considering reports published worldwide since 1993, our analysis showed the use of RUCAM for causality assessment in 95,885 cases of liver injury including 81,856 cases of idiosyncratic DILI and 14,029 cases of HILI. Among the top countries providing RUCAM based DILI cases were, in decreasing order, China, the US, Germany, Korea, and Italy, with China, Korea, Germany, India, and the US as the top countries for HILI. Conclusion: Since 1993 RUCAM is certainly the most widely used method to assess causality in IDILI and HILI. This should encourage practitioner, experts, and regulatory agencies to use it in order to reinforce their diagnosis and to take sound decisions.
In the past, the genetically diabetic-obese diabetes/diabetes (db/db) and obese/obese (ob/ob) mouse strains were used to investigate mechanisms of diabetes-impaired wound healing. Here we determined patterns of skin repair in genetically normal C57Bl/6J mice that were fed using a high fat diet (HFD) to induce a diabetes-obesity syndrome. Wound closure was markedly delayed in HFD-fed mice compared to mice which had received a standard chow diet (CD). Impaired wound tissue of HFD mice showed a marked prolongation of wound inflammation. Expression of vascular endothelial growth factor (VEGF) was delayed and associated with the disturbed formation of wound margin epithelia and an impaired angiogenesis in the reduced granulation tissue. Normal wound contraction was retarded and disordered. Wound disorders in obese C57Bl/6J mice were paralleled by a prominent degradation of the inhibitor of NFκB (IκB-α) in the absence of an Akt activation. By contrast to impaired wound conditions in ob/ob mice, late wounds of HFD mice did not develop a chronic inflammatory state and were epithelialized after 11 days of repair. Thus, only genetically obese and diabetic ob/ob mice finally developed chronic wounds and therefore represent a better suited experimental model to investigate diabetes-induced wound healing disorders.
Background: Patient information materials and decision aids are essential tools for helping patients make informed decisions and share in decision-making. The aim of this study was to investigate the quality of the written patient information materials available at general practices in Styria, Austria.
Methods: We asked general practitioners to send in all patient information materials available in their practices and to answer a short questionnaire. We evaluated the materials using the Ensuring Quality Information for Patients (EQIP-36) instrument.
Results: A total of 387 different patient information materials were available for quality assessment. These materials achieved an average score of 39 out of 100. The score was below 50 for 78% of all materials. There was a significant lack of information on the evidence base of recommendations. Only 9 % of the materials provided full disclosure of their evidence sources. We also found that, despite the poor quality of the materials, 89% of general practitioners regularly make active use of them during consultations with patients.
Conclusion: Based on international standards, the quality of patient information materials available at general practices in Styria is poor. The vast majority of the materials are not suitable as a basis for informed decisions by patients. However, most Styrian general practitioners use written patient information materials on a regular basis in their daily clinical practice. Thus, these materials not only fail to help raise the health literacy of the general population, but may actually undermine efforts to enable patients to make shared informed decisions. To increase health literacy, it is necessary to make high quality, evidence-based and easy-to-understand information material available to patients and the public. For this, it may be necessary to set up a centralized and independent clearinghouse.
Die mediane Sternotomie ist in der Kardio-Chirurgie der wichtigste Zugang zum Herzen. Postoperative Wundheilungsstörungen in diesem Bereich sind seltene, jedoch gefürchtete Komplikationen, die zu schwerwiegenden Konsequenzen führen können. Die zunächst einfache oberflächliche Wundheilungsstörung kann sich verkomplizieren und über eine Osteomyelitis des Sternums zu einer Mediastinitis mit eventuell letalen Konsequenzen führen. Aktuell geben unterschiedliche Autoren eine Inzidenz von 1 % – 8 % an. Die Mortalität der Sternumosteitis wird dabei zwischen 10 % und 30 % angegeben. Wird der Pathomechanismus der Erkrankung zu einem Algorithmus zusammengefasst, so kann sich über einen descendierenden Verlauf eine oberflächliche Wundheilungsstörung zu einer Sternumosteitis und schließlich zu einer Mediastinitis entwickeln. Natürlich kann sich die Infektion auch ascendierend ausbreiten. Über eine Infektion des Mediastinums oder des Sternums kann sich eine Infektion über die Weichteile bis an die Körperoberfläche ausdehnen. Der typische Patient, der eine Wundheilungsstörung erleidet ist in aller Regel polymorbide. Besonders bei diesem Patientenkollektiv sind eine kurze Behandlungsdauer und eine schnelle, postoperative Rehabilitation für das Ergebnis vorrangig. Ist der gewählte Therapieplan nicht aggressiv genug, so kommt es häufig zu chronischen Erkrankungen mit Fistelungen und einem konsekutiven Fortschreiten der Infektion in Weichteilen und Knochen. Neben medizinischen Gesichtspunkten kommen auch der wirtschaftliche Aspekte zu tragen. Wiederholte Operationen, lange Liegezeiten auf der Intensivstation und wiederholte sowie langwierige Rehabilitationsphasen sind nur wenige Beispiele der kostenintensiven Therapiebestandteile. Aufgrund der schwer beherrschbaren Infektsituation ist eine konsequent aggressive, definitive und zuverlässige Versorgung der infizierten Wunden von höchster Priorität. Dabei spielt es keine Rolle, ob es sich dabei um Wundheilungsstörungen im Bereich der Haut- und Unterhautweichteile, des Sternums oder des Mediastinums handelt. Zur Behandlung der Wundheilungsstörung nach medianer Sternotomie existieren verschiedene Behandlungsansätze und Therapieoptionen. Diese reichen von einfachen Debridements mit Sekundärvernähungen über Spül-Saug-Drainagen mit Antibiotika versetzen Lösungen bis hin zu Sternumteilresektionen, Vakuumverbänden und komplexen Lappenplastiken. ...
In der vorliegenden Arbeit werden die angewendeten Kräfte, die von drei Behandlergruppen (Studenten, Zahnärzte und Spezialisten) während der lateralen Kondensation von Guttapercha aufgebracht werden, in Abhängigkeit vom Qualifikationsgrad des Behandlers und der unterschiedlichen Kanalkonizität untersucht. Die Behandlergruppen bestehen aus jeweils 12 Personen. Jeder Proband füllt in folgender Reihenfolge einen vorgefertigten Kunststoffkanal der Konizität .02, .04, .06 und p30 (ProTaper F3) nach der Methode der lateralen Kondensation unter Verwendung von AH plus Sealer und Guttapercha. Dabei werden von einem Kraftsensor die entstehenden vertikalen Kräfte gemessen, aufgezeichnet und dann mittels geeigneter Software hinsichtlich folgender Ergebnisgrößen ausgewertet: • Absolute Maxima der angewendeten Kraft bei Wurzelkanalfüllung • Durchschnittswerte der angewendeten Kraft • Häufigkeitsverteilung der Kraftmesswerte • Zugkraftwerte (Minima der angewendeten Kraft) • Streuungsmaße (Standardabweichung und Varianz) • Anzahl der auftretenden Kraftspitzen pro Kanalfüllung • zugrunde liegende probandenspezifische Füllmuster • Anzahl der zur Kanalfüllung verwendeten Guttaperchastifte • benötigte Kanalfüllzeit Folgende Ergebnisse sind erarbeitet worden: 1. Die 36 Probanden haben durchschnittliche Maximalwerte von 1,10-2,12 kg eingesetzt: Spezialisten wendeten im Mittel 2,12 kg an, Studenten 1,48 kg und Zahnärzte 1,10 kg. 2. Die Durchschnittswerte der eingesetzten Lasten der 36 Probanden liegen zwischen 0,51-0,92 kg: Spezialisten wendeten im Mittel 0,92 kg an, Studenten 0,65 kg und Zahnärzte 0,51 kg. Auch hier kondensieren die Spezialisten mit höheren Kraftwerten. 3. Mit ansteigender Konizität werden von allen Behandlergruppen signifikant mehr Guttaperchastifte in den Kanal eingebracht. Zahnärzte tendieren dazu, weniger Guttaperchastifte zu verwenden und die Kanäle schneller zu füllen, als die Probanden der anderen beiden Gruppen.
Hintergrund und Fragestellung Medizinische Fehlerberichtssysteme dienen als Instrument zum Erfassen von Daten aus dem Bereich der Patientensicherheit und sollen das Lernen aus den berichteten Fehlern ermöglichen. Erfahrungen hierzu liegen hauptsächlich aus dem Umfeld der Kliniken vor, für den ambulanten bzw. hausärztlichen Bereich gibt es noch wenige Kenntnisse. Ziel dieser Arbeit ist die Erstellung und Erprobung eines praxistauglichen, deutschsprachigen Fehlerberichts- und Lernsystems für hausärztlich tätige Mediziner/innen. Weiterhin soll die Verwendung und Nutzerakzeptanz nach einem Jahr Betrieb untersucht werden. Methoden Zur Festlegung der Eigenschaften des zu erstellenden Systems wurden drei existierende Berichtssysteme analysiert, eine Befragung der Teilnehmer einer Pilotstudie (PCISME) durchgeführt sowie juristische Aspekte untersucht. Das System wurde mit Hilfe etablierter Softwareentwicklungsmethoden erstellt. Nach einem Jahr Laufzeit wurden Daten zur Verwendung des Systems ermittelt und die veröffentlichten Berichte und abgegebenen Benutzerkommentare insbesondere bezüglich Aussagen zum Feedbacksystem und zur Nutzerfreundlichkeit untersucht. Ergebnisse Im Oktober 2004 wurde mit „Jeder Fehler zählt“ das erste Fehlerberichts- und Lernsystem für Hausärzte in Deutschland freigegeben. Es ist ein komplett offenes, freiwilliges, anonymes und externes System. Die Eingabe der Fehlerberichte erfolgt über ein im Internet frei zugängliches Formular. Ausgesuchte Fehlerberichte werden von wissenschaftlichen Mitarbeitern kommentiert und veröffentlicht. Benutzer können zu den Berichten und in einem Diskussionsforum ebenfalls Kommentare eingeben. Weiterhin werden Berichte und Analysen in Fachzeitschriften publiziert. Im ersten Jahr gingen 149 Berichte ein, laut Aussagen von 23 Benutzern ist das Berichten einfach durchzuführen. Es wurden 11 „Fehler des Monats“ veröffentlicht, zu denen insgesamt 123 Kommentare abgegeben wurden. Zu 43 veröffentlichten „Fehler der Woche“ wurden 146 Kommentare abgegeben. Im Diskussionsforum wurden 46 Einträge vorgenommen. Die Beiträge der Benutzer sind zum weit überwiegenden Teil konstruktiv und sachbezogen und enthalten häufig konkrete Tipps und Hinweise zur Vermeidung von Fehlern. In deutschen und österreichischen Fachzeitschriften wurden insgesamt 11 Berichte publiziert. Diskussion Das System führt Eigenschaften der untersuchten Systeme mit eigenen Ansätzen zusammen. Da wegen der Anonymität der Berichtenden keine Möglichkeit zum Nachfragen besteht, ist für eine gute Verwertbarkeit der Berichte eine möglichst hochwertige textuelle Beschreibung der Ereignisse notwendig. Diese war in der überwiegenden Zahl der Berichte gegeben. Aus Sicht der aktiven Nutzer stellt das System grundsätzlich einen wertvollen Beitrag zur Qualitäts- und Fehlerkultur dar. Schlussfolgerung Die regelmäßige Nutzung der Internetseiten, die dort abgegebenen Kommentare und die Veröffentlichungen der Berichte in den Zeitschriften zeigen, dass auch einzelne Berichte das Lernen aus Fehlern ermöglichen können. „Jeder Fehler zählt“ kann als hypothesengenerierendes System mit vielfältigen Ansätzen zur weiteren Forschung aufgefasst werden. Bereits im ersten Jahr des Betriebs konnten wertvolle Erfahrungen gesammelt werden, die in die kontinuierliche Weiterentwicklung des Systems einfließen. Diese fokussiert auf Verbesserungen der Feedbackmethoden sowie auf eine höhere Nutzung des Systems.
