Mesenchymale Stromazellen stellen derzeit eine der vielversprechensten Zellpopulationen dar und werden in Hinblick auf ihre zahlreichen Fähigkeiten und Eigenschaften intensiv erforscht. In dieser Arbeit konzentrierten wir uns auf das für die Behandlung von immunvermittelten Erkrankungen wertvolle immunmodulatorische Potential der mesenchymalen Stromazellen. Dieses ist für standardmäßigplastikadhärent isolierte MSCs vielfach belegt. Der große Nachteil dieser Isolierungsmethode jedoch, ist die Gewinnung nur sehr heterogener Zellpopulationen, was das klonogene, proliferative und möglicherweise auch immunsuppressive Potential dieser Zellen betrifft. Deshalb schien es uns sinnvoll, die Eigenschaften mesenchymaler Stromazellen zu untersuchen, welche nach einer alternativen Selektionsprozedur, die sich des Oberflächenmarkers CD271 (LNGFR) bedient, und homogenere Zellpopulationen von MSCs hervorbringt. Den auf diese Art isolierten und angereicherten MSCs wird in der Literatur bereits ein erhöhtes klonogenes und proliferatives Potential bescheinigt, welches wir im Rahmen dieser Arbeit überprüften und schließlich auch bestätigen konnten. Im Zentrum dieser Dissertation standen jedoch Untersuchungen zum immunsuppressiven Potential dieser alternativselektierten MSCs, welches bis Dato noch nicht erforscht war. Zur Beantwortung der Frage, ob CD271-selektierte MSCs ein vergleichbares oder möglicherweise durch ihre Homogenität erreichtes, verbessertes immunsuppressives Potential auf Immunzellen entfalten, kultivierten wir diese Zellen mit auf unterschiedliche Weise stimulierten PBMNCs und evaluierten darunter deren Proliferationspotential. Wir fanden heraus, dass CD271+-MSCs in bestimmten Konzentrationen ein im Vergleich mit PA-MSCs erhöhtes immunsuppressives Potential auf allogen stimulierte PBMNCs entfalten und dabei in keinem unserer Versuche den standardmäßig selektierten MSCs unterlegen waren. Bei einem Anteil von 50% MSCs an der Zellsuspension vermögen CD271+- MSCs die Proliferation in stärkerem Maße zu inhibieren, als es PA-MSCs tun. Werden MSCs und PBMNCs in gleichem Verhältnis kultiviert, unterscheidet sich das Maß der Inhibition durch die beiden Zelltypen nicht signifikant. Betrug der Anteil der eingesetzten MSCs an der Zellsuspension nur geringe 1%, stimulierten CD271+-MSCs nicht die Proliferation der Lymphozyten, während es unter dem Einfluss von PA-MSCs zu signifikanter Proliferationssteigerung kam. Insgesamt stellte sich das immunmodulatorische Potential der CD271+-MSCs, genau wie das der PA-MSCs als dosisabhängig dar. Je höher der MSC-Anteil an der Zellsuspension, desto höher war das Suppressionspotential auf die Lymphozytenproliferation. Vergleichbare Ergebnisse lieferten Versuche, in denen MSCs zusammen mit IL-2 stimulierten allogenen PBMNCs eingesetzt wurden, was beweist, dass CD271+-MSCs auch maximal stimulierte PBMNCs signifikant proliferativ hemmen können. In Versuchen mit mitogen stimulierten PBMNCs konnten wir nachweisen, dass CD271+-MSCs auch die Proliferation von zuvor mit PHA, ConA und IL-2 stimulierten PBMNCs inhibieren. Im Gegensatz zu PA-MSCs vermochten sie aber nicht, die proliferationsfördernde Wirkung des Staphylokokken-Enterotoxin-B (SEB) aufzuheben. Um den Mechanismen der immunsuppressiven Fähigkeit mesenchymaler Stromazellen näher zu kommen, untersuchten wir drei erst kürzlich beschriebene mögliche Mediatoren der MSCvermittelten Immunmodulation. Wir analysierten die Wirkung mesenchymaler Stromazellen auf regulatorische T-Zellen, auf die Produktion von PGE2 und NO. Unter der Präsenz der MSCs kam es in der Population der allogen-stimulierten PBMNCs zu keiner signifikanten Zu- oder Abnahme an regulatorischen CD4+/CD25+-Zellen. Innerhalb der Population der regulatorischen T-Zellen kam es jedoch unter dem Einfluss der CD271+-MSCs zu einer signifikanten Zunahme der primitiveren Subpopulation CD4+CD25+CD45RA+ T-regulatorischer Zellen, welcher in der Literatur eine robuste suppressive Aktivität nach in vitro-Expansion bescheinigt wird. NO konnten wir durch unsere Untersuchungen als möglichen Mediator der Immunmodulation ausschließen. Es ergaben sich keine signifikanten Unterschiede für den NO-Gehalt in PBMNC-Kulturen, die in Anwesenheit oder Abwesenheit von MSCs inkubiert worden waren. Auch NO-Blockierungsversuche erbrachten keinen Effekt auf die Zellproliferation. Jedoch fanden wir eine signifikant positive Korrelation zwischen der PGE2-Konzentration und dem inhibitorischen Effekt von CD271+-MSCs auf die Proliferation der PBMNCs. Dass das PGE2 in unseren Experimenten das wichtigste Vermittlermolekül des immunsuppressiven Effekts von CD271+-MSCs darstellt, konnten wir zusätzlich in Versuchen mit dem PGE2-Syntheseinhibitor Indomethacin beweisen. In Zusammenschau mit Daten anderer Arbeitsgruppen ist insgesamt die Schlussfolgerung zulässig, dass es sich bei der Immunmodulation durch MSCs um einen komplexen Mechanismus handelt, der sowohl lösliche Faktoren, als auch Immunzellen einschließt und welcher weiterer Erforschung bedarf. Anhand unserer Untersuchungen konnten wir zeigen, dass CD271-selektierte MSCs neben der verbessert klonogenen und proliferativen Aktivität eine teilweise erhöhte und mindestens ebenbürtige immunsuppressive Fähigkeit, verglichen mit den nach Standard angereicherten MSCs, aufweisen. Letztlich befürworten und bestärken unsere Ergebnisse einen Einsatz CD271-angereicherter mesenchymaler Stromazellen zur Behandlung immunvermittelter Erkrankungen und versprechen eine vielseitige Verwendung dieser Zellen in Zukunft. Die vorliegende Arbeit leistet einen wichtigen Beitrag zur notwendigen Standardisierung und Weiterentwicklung der Gewinnung und Anreicherung funktionsfähiger mesenchymaler Stromazellen und zum sicheren und raschen Einsatz dieser Zellen im klinischen Gebrauch. Weil es sich bei den vorgestellten Ergebnissen allerdings ausschließlich um ex vivo Experimente handelt, sind weitergehende Untersuchungen im Tierexperiment notwendig.
Background: Because Endomyocardial Biopsy has low sensitivity of about 20%, it can be performed near to myocardium that presented as Late Gadolinium Enhancement (LGE) in cardiovascular magnetic resonance (CMR). However the important issue of comparing topography of CMR and histological findings has not yet been investigated. Thus the current study was performed using an animal model of myocarditis. Results: In 10 male Lewis rats Experimental Autoimmune myocarditis was induced, 10 rats served as control. On day 21 animals were examined by CMR to compare topographic distribution of LGE to histological inflammation. Sensitivity, specificity, positive and negative predictive values for LGE in diagnosing myocarditis were determined for each segment of myocardium. Latter diagnostic values varied widely depending on topographic distribution of LGE and inflammation as well as on the used CMR sequence. Sensitivity of LGE was up to 76% (left lateral myocardium) and positive predictive values were up to 85% (left lateral myocardium), whereas sensitivity and positive predictive value dropped to 0 - 33% (left inferior myocardium). Conclusions: Topographic distribution of LGE and histological inflammation seem to influence sensitivity, specifity, positive and negative predictive values. Nevertheless, positive predictive value for LGE of up to 85% indicates that Endomyocardial Biopsy should be performed "MR-guided". LGE seems to have greater sensitivity than Endomyocardial Biopsy for the diagnosis of myocarditis.
Die mikroskopische Anatomie sowie die pathologische Anatomie bilden einen wichtigen Baustein der Ausbildung von Medizin- und Zahnmedizinstudenten. Da die zeitlichen und personellen Kapazitäten in den Instituten für Pathologie bzw. Anatomie keine vierundzwanzigstündige Betreuung erlauben, bestand der Wunsch, ein zeit- und ortsunabhängiges webbasiertes und interaktives multimediales Lernprogramm zu entwickeln. Es sollte den Studierenden beim Erlernen der Histologie und Histopathologie helfen und die Präsenzveranstaltungen am Mikroskop und Vorlesungen Ergänzen. Das Lernprogramm wurde mit zahlreichen hochauflösenden Bildern aller histologischen Präparate, die Bestandteil der Histologie- und Histopathologiekurse sind, in verschiedenen Vergrößerungen ausgestattet und in die Kurse des Frankfurter Curriculum für Medizin- und Zahnmedizin integriert. Darüber hinaus wurden alle histologischen Webseiten mit den entsprechenden histopathologischen Seiten verlinkt und umgekehrt. Ein Autorensystem zur Aktualisierung, Korrektur und Erweiterung der Programme wurde entwickelt. Mit der Hilfe der Evaluation durch die Studierenden wurde das System im Verlauf stark verbessert. Die hohe Zahl der Zugriffe auf die Website über das Internet (500.000 – 800.000 im Monat) weist auf die hohe Akzeptanz und Beliebtheit des Programms hin. Im wesentlichen zeigt die vorliegende Arbeit, dass internet-basierte Multimedia-Projekte von Studierenden verwendet werden und ihnen helfen, komplexe Sachverhalte der Histologie und Histopathologie zu erlernen. Die enge Verzahnung zwischen histologischen und histopathologischen Themen und Präparaten ermöglicht Studierenden der vorklinischen Fächer, über krankheitsbedingte Veränderungen zu erfahren, Studierende der klinischen Fächer hingegen können das histologische Basiswissen rekapitulieren. Das Autorensystem kann zur Entwicklung weiterer webbasierter Anwendungen, wie z. B. Virtuelle Mikroskopie oder Wissensprüfungsprogramme für die Histologie/Histopathologie, verwendet werden.
Laut BQS (Bundesgeschäftsstelle Qualitätssicherung) nimmt die Anzahl der durchgeführten Hüftendoprothesenerstimplantationen kontinuierlich zu (2006: 146.853; 2008: 157.350), bedingt nicht zuletzt durch die demographische Entwicklung (zunehmende Alterung der Gesellschaft). Die auftretenden Komplikationen nach solchen Operationen legen es nahe, an einer weiteren Optimierung des Behandlungsverfahrens zu arbeiten. Dabei wurde bisher kaum untersucht, inwieweit bei einer Hüftprothesenimplantation das Rotationsprofil der unteren Extremität die Prothesenauswahl bzw. die Prothesengestalt beeinflussen sollte. Das Resultat der Messung anthropometrischer Rotationsverhältnisse durch Bestimmung des Antetorsionswinkels (AT oder ATWinkel) und Unterschenkelrotationswinkels (UR oder UR-Winkel) an der unteren Extremität und deren Inbezugsetzung zueinander wird in dieser Studie „Rotationsprofil“ genannt. Ein solches Rotationsprofil wird allerdings in der CT-basierten Individualendoprothetik des Hüftgelenks routinemäßig erhoben und sein Ergebnis in die Prothesenrekonstruktion oder -auswahl eingearbeitet. Der Grund für diese Vorgehensweise liegt in der Überlegung, dass es – bei einer wie üblich vorgenommenen Ausrichtung z. B. einer Geradschaftprothese an der Kniekondylenebene – unter Umständen postoperativ zu einer tatsächlichen Retrotorsion bzw. zu Einschränkungen des Bewegungsumfangs im Hüftgelenk kommen kann. Theoretisch ist in extremen Fällen nicht nur eine Luxation des Hüftkopfs vorstellbar, sondern auch eine mittelfristig induzierte Lockerung der Prothese, bedingt durch eine suboptimale Anpassung des ATWinkels im Rahmen der Planung des Prothesendesigns, und damit eine veränderte Krafteinleitung in das proximale Femur. In der hier vorgestellten Studie wird anhand eines größeren Patientenkollektivs, bei dem die Indikation für eine Hüftgelenksprothese aufgrund einer primären oder sekundären Coxarthrose gestellt worden war, das Rotationsprofil an der betroffenen Extremität im CT ermittelt. In dem Patientenkollektiv sollte zunächst eine mögliche geschlechtsspezifische Beziehung zwischen den Variablen AT und UR nachgewiesen werden. Erstmalig wurde eine Analyse des Rotationsprofils in verschiedenen Subgruppen hinsichtlich des Alters (vier Subgruppen) und der Größe des Antetorsionswinkels (fünf Subgruppen) durchgeführt. Es wurde die Hypothese überprüft, ob ein hoher AT-Winkel mit einem hohen UR-Winkel korreliert.
To date it is not clear at which stage of differentiation mature T cell leukaemia/lymphoma is initiated. Previous studies in our group showed that mature T cells are relatively resistant to transformation. We wanted to further investigate the transformation potential of NPM-ALK, p21SNFT and the viral oncoprotein Tax on mature T cells. First, we analyzed the effects on T cell growth in vitro after transducing human T cell lines with gammaretroviral vectors encoding these genes. No growth or proliferation promoting effect of all three genes was observed. In the second part of the project, we transduced murine, mature T cells and/or haematopoietic stem cells (HPCs/HSCs) and transplanted these cells into Rag-1 deficient recipients. All mice transplanted with NPM-ALK transduced monoclonal mature T cells (OT-1) developed leukaemia/lymphoma. In contrast, only few NPM-ALK transduced polyclonal T cell and HPC/HSC transplanted mice developed leukaemia/lymphoma. From the p21SNFT group, only two mice transplanted with transduced OT-1 T cells developed leukaemia/lymphoma, which showed high eGFP and interestingly CD19 expression. No malignancies were observed in Tax transplanted animals so far. Furthermore, the recipients do not show any eGFP marking in the periphery. In conclusion, our results show that compared to polyclonal T cells, monoclonal T cells are transformable after gammaretroviral transfer of NPM-ALK and p21SNFT.
Introduction: It has been proposed that individual genetic variation contributes to the course of severe infections and sepsis. Recent studies of single nucleotide polymorphisms (SNPs) within the endotoxin receptor and its signaling system showed an association with the risk of disease development. This study aims to examine the response associated with genetic variations of TLR4, the receptor for bacterial LPS, and a central intracellular signal transducer (TIRAP/Mal) on cytokine release and for susceptibility and course of severe hospital acquired infections in distinct patient populations. Methods: Three intensive care units in tertiary care university hospitals in Greece and Germany participated. 375 and 415 postoperative patients and 159 patients with ventilator associated pneumonia (VAP) were included. TLR4 and TIRAP/Mal polymorphisms in 375 general surgical patients were associated with risk of infection, clinical course and outcome. In two prospective studies, 415 patients following cardiac surgery and 159 patients with newly diagnosed VAP predominantly caused by Gram-negative bacteria were studied for cytokine levels in-vivo and after ex-vivo monocyte stimulation and clinical course. Results: Patients simultaneously carrying polymorphisms in TIRAP/Mal and TLR4 and patients homozygous for the TIRAP/Mal SNP had a significantly higher risk of severe infections after surgery (odds ratio (OR) 5.5; confidence interval (CI): 1.34 - 22.64; P = 0.02 and OR: 7.3; CI: 1.89 - 28.50; P < 0.01 respectively). Additionally we found significantly lower circulating cytokine levels in double-mutant individuals with ventilator associated pneumonia and reduced cytokine production in an ex-vivo monocyte stimulation assay, but this difference was not apparent in TIRAP/Mal-homozygous patients. In cardiac surgery patients without infection, the cytokine release profiles were not changed when comparing different genotypes. Conclusions: Carriers of mutations in sequential components of the TLR signaling system may have an increased risk for severe infections. Patients with this genotype showed a decrease in cytokine release when infected which was not apparent in patients with sterile inflammation following cardiac surgery.
BACKGROUND: Parkinson's disease (PD), the second most frequent neurodegenerative disorder at old age, can be caused by elevated expression or the A53T missense mutation of the presynaptic protein alpha-synuclein (SNCA). PD is characterized pathologically by the preferential vulnerability of the dopaminergic nigrostriatal projection neurons. METHODOLOGY/PRINCIPAL FINDINGS: Here, we used two mouse lines overexpressing human A53T-SNCA and studied striatal dysfunction in the absence of neurodegeneration to understand early disease mechanisms. To characterize the progression, we employed young adult as well as old mice. Analysis of striatal neurotransmitter content demonstrated that dopamine (DA) levels correlated directly with the level of expression of SNCA, an observation also made in SNCA-deficient (knockout, KO) mice. However, the elevated DA levels in the striatum of old A53T-SNCA overexpressing mice may not be transmitted appropriately, in view of three observations. First, a transcriptional downregulation of the extraneural DA degradation enzyme catechol-ortho-methytransferase (COMT) was found. Second, an upregulation of DA receptors was detected by immunoblots and autoradiography. Third, extensive transcriptome studies via microarrays and quantitative real-time RT-PCR (qPCR) of altered transcript levels of the DA-inducible genes Atf2, Cb1, Freq, Homer1 and Pde7b indicated a progressive and genotype-dependent reduction in the postsynaptic DA response. As a functional consequence, long term depression (LTD) was absent in corticostriatal slices from old transgenic mice. CONCLUSIONS/SIGNIFICANCE: Taken together, the dysfunctional neurotransmission and impaired synaptic plasticity seen in the A53T-SNCA overexpressing mice reflect early changes within the basal ganglia prior to frank neurodegeneration. As a model of preclinical stages of PD, such insights may help to develop neuroprotective therapeutic approaches.
The presynaptic protein alpha-synuclein has received much attention because its gain-of-function is associated with Parkinson’s disease. However, its physiological function is still poorly understood. We studied brain regions of knock-out mice at different ages with regard to consistent upregulations of the transcriptome and focused on glyoxalase I (GLO1). The microarray data were confirmed in qPCR, immunoblot, enzyme activity, and behavior analyses. GLO1 induction is a known protective cellular response to glucose stress, representing efforts to decrease toxic levels of methylglyoxal (MG), glyoxal and advanced glycation endproducts (AGEs). Mass spectrometry quantification demonstrated a ubiquitous increase in MG and fructosyl-lysine as consequences of glucose toxicity, and consistent enhancement of certain AGEs. Thus, GLO1 induction in KO brain seems insufficient to prevent AGE formation. In conclusion, the data demonstrate GLO1 expression and glycation damage to be induced by alpha-synuclein ablation. We propose that wild-type alpha-synuclein modulates brain glucose metabolism.
Background: Occupational demands of educators are not very well researched. Nevertheless their work is subject to several requirements. Whether these demands have an effect on the work ability and the health status of employees has also not been examined. Furthermore it is unclear if the ownership type of day care centres have an influence on job satisfaction and work ability of the pedagogical staff and what kind of resources do exist. Previous studies were mainly based on questionnaire data. Objective data does not exist. Therefore the aim of this investigation is to collect precise data relating to work of educators.
Methods: Effects of different types of ownership of day care centres on job satisfaction and work ability of educators will be assessed with the help of objective real time studies in combination with multi-level psycho diagnostic measurements.
Discussion: The present study is the first of its kind. Up to now there are no computer-based real time studies on workflow of pedagogical staff with regard to assess their work-related stress. Following an exhaustive documentation of educators work processes the day-to-day task can be estimated and approaches for prevention can be developed. This can substantially contribute to an overall improvement of child care in Germany.
Background: Diseases associated with smoking are a foremost cause of premature death in the world, both in developed and developing countries. Eliminating smoking can do more to improve health and prolong life than any other measure in the field of preventive medicine. Today's medical students will play a prominent role in future efforts to prevent and control tobacco use.
Methods: A cross-sectional, self-administered, anonymous survey of fifth-year medical students in Berlin, Germany was conducted in November 2007. The study explored the prevalence of smoking among medical students. We assessed their current knowledge regarding tobacco dependence and the effectiveness of smoking cessation methods. Students' perceived competence to counsel smokers and promote smoking cessation treatments was also explored. Analyses were based on responses from 258 students (86.6% response rate).
Results: One quarter of the medical students surveyed were current smokers. The smoking rate was 22.1% among women, 32.4% among men. Students underestimated smoking-related mortality and the negative effect of smoking on longevity. A considerable number of subjects erroneously assumed that nicotine causes coronary artery disease. Students' overall knowledge of the effectiveness of smoking cessation methods was inadequate. Only one third of the students indicated that they felt qualified to counsel patients about tobacco dependence.
Conclusions: This study reveals serious deficiencies in knowledge and counseling skills among medical students in our sample. The curriculum of every medical school should include a tobacco module. Thus, by providing comprehensive training in nicotine dependence interventions to medical students, smokers will have access to the professional expertise they need to quit smoking.
