Allogeneic hematopoietic cell transplantation (allo-HCT) is increasingly used in older myelofibrosis (MF) patients, but its risk/benefit ratio compared to non-transplant approaches has not been evaluated in this population. We analyzed the outcomes of allo-HCT in 556 MF patients aged ≥65 years from the EBMT registry, and determined the excess mortality over the matched general population of MF patients ≥65 years managed with allo-HCT (n = 556) or conventional drug treatment (n = 176). The non-transplant cohort included patients with intermediate-2 or high risk DIPSS from the Spanish Myelofibrosis Registry. After a median follow-up of 3.4 years, the estimated 5-year survival rate, non-relapse mortality (NRM), and relapse incidence after transplantation was 40%, 37%, and 25%, respectively. Busulfan-based conditioning was associated with decreased mortality (HR: 0.7, 95% CI: 0.5–0.9) whereas the recipient CMV+/donor CMV- combination (HR: 1.7, 95% CI: 1.2–2.4) and the JAK2 mutated genotype (HR: 1.9, 95% CI: 1.1–3.5) predicted higher mortality. Busulfan-based conditioning correlated with improved survival due to less NRM, despite its higher relapse rate when compared with melphalan-based regimens. Excess mortality was higher in transplanted patients than in the non-HCT cohort in the first year of follow-up (ratio: 1.93, 95% CI: 1.13–2.80), whereas the opposite occurred between the fourth and eighth follow-up years (ratio: 0.31, 95% CI: 0.18–0.53). Comparing the excess mortality of the two treatments, male patients seemed to benefit more than females from allo-HCT, mainly due to their worse prognosis with non-transplant approaches. These findings could potentially enhance counseling and treatment decision-making in elderly transplant-eligible MF patients.
The transcription factor vitamin D receptor (VDR) is the high affinity nuclear target of the biologically active form of vitamin D3 (1,25(OH)2D3). In order to identify pure genomic transcriptional effects of 1,25(OH)2D3, we used VDR cistrome, transcriptome and open chromatin data, obtained from the human monocytic cell line THP-1, for a novel hierarchical analysis applying three bioinformatics approaches. We predicted 75.6% of all early 1,25(OH)2D3-responding (2.5 or 4 h) and 57.4% of the late differentially expressed genes (24 h) to be primary VDR target genes. VDR knockout led to a complete loss of 1,25(OH)2D3–induced genome-wide gene regulation. Thus, there was no indication of any VDR-independent non-genomic actions of 1,25(OH)2D3 modulating its transcriptional response. Among the predicted primary VDR target genes, 47 were coding for transcription factors and thus may mediate secondary 1,25(OH)2D3 responses. CEBPA and ETS1 ChIP-seq data and RNA-seq following CEBPA knockdown were used to validate the predicted regulation of secondary vitamin D target genes by both transcription factors. In conclusion, a directional network containing 47 partly novel primary VDR target transcription factors describes secondary responses in a highly complex vitamin D signaling cascade. The central transcription factor VDR is indispensable for all transcriptome-wide effects of the nuclear hormone.
In 2004, Germany introduced a program based on voluntary contracting to strengthen the role of general practice care in the healthcare system. Key components include structured management of chronic diseases, coordinated access to secondary care, data-driven quality improvement, computerized clinical decision-support, and capitation-based reimbursement. Our aim was to determine the long-term effects of this program on the risk of hospitalization of specific categories of high-risk patients. Based on insurance claims data, we conducted a longitudinal observational study from 2011 to 2018 in Baden-Wuerttemberg, Germany. Patients were assigned to one or more of four open cohorts (in 2011, elderly, n = 575,363; diabetes mellitus, n = 163,709; chronic heart failure, n = 82,513; coronary heart disease, n = 125,758). Adjusted for key patient characteristics, logistic regression models were used to compare the hospitalization risk of the enrolled patients (intervention group) with patients receiving usual primary care (control group). At the start of the study and throughout long-term follow-up, enrolled patients in the four cohorts had a lower risk of all-cause hospitalization and ambulatory, care-sensitive hospitalization. Among patients with chronic heart failure and coronary heart disease, the program was associated with significantly reduced risk of cardiovascular-related hospitalizations across the eight observed years. The effect of the program also increased over time. Over the longer term, the results indicate that strengthening primary care could be associated with a substantial reduction in hospital utilization among high-risk patients.
Objective: To evaluate the incidence and risk factors of generalized convulsive seizure (GCS)-related fractures and injuries during video-EEG monitoring.
Methods: We analyzed all GCSs in patients undergoing video-EEG-monitoring between 2007 and 2019 at epilepsy centers in Frankfurt and Marburg in relation to injuries, falls and accidents associated with GCSs. Data were gathered using video material, EEG material, and a standardized reporting form.
Results: A total of 626 GCSs from 411 patients (mean age: 33.6 years; range 3–74 years; 45.0% female) were analyzed. Severe adverse events (SAEs) such as fractures, joint luxation, corneal erosion, and teeth loosening were observed in 13 patients resulting in a risk of 2.1% per GCS (95% CI 1.2–3.4%) and 3.2% per patient (95% CI 1.8–5.2%). Except for a nasal fracture due to a fall onto the face, no SAEs were caused by falls, and all occurred in patients lying in bed without evidence of external trauma. In seven patients, vertebral body compression fractures were confirmed by imaging. This resulted in a risk of 1.1% per GCS (95% CI 0.5–2.2%) and 1.7% per patient (95% CI 0.8–3.3%). These fractures occurred within the tonic phase of a GCS and were accompanied by a characteristic cracking noise. All affected patients reported back pain spontaneously, and an increase in pain on percussion of the affected spine section.
Conclusions: GCSs are associated with a substantial risk of fractures and shoulder dislocations that are not associated with falls. GCSs accompanied by audible cracking, and resulting in back pain, should prompt clinical and imaging evaluations.
Borrelia miyamotoi, a relapsing fever spirochete transmitted by Ixodid ticks causes B. miyamotoi disease (BMD). To evade the human host´s immune response, relapsing fever borreliae, including B. miyamotoi, produce distinct variable major proteins. Here, we investigated Vsp1, Vlp15/16, and Vlp18 all of which are currently being evaluated as antigens for the serodiagnosis of BMD. Comparative analyses identified Vlp15/16 but not Vsp1 and Vlp18 as a plasminogen-interacting protein of B. miyamotoi. Furthermore, Vlp15/16 bound plasminogen in a dose-dependent fashion with high affinity. Binding of plasminogen to Vlp15/16 was significantly inhibited by the lysine analog tranexamic acid suggesting that the protein–protein interaction is mediated by lysine residues. By contrast, ionic strength did not have an effect on binding of plasminogen to Vlp15/16. Of relevance, plasminogen bound to the borrelial protein cleaved the chromogenic substrate S-2251 upon conversion by urokinase-type plasminogen activator (uPa), demonstrating it retained its physiological activity. Interestingly, further analyses revealed a complement inhibitory activity of Vlp15/16 and Vlp18 on the alternative pathway by a Factor H-independent mechanism. More importantly, both borrelial proteins protect serum sensitive Borrelia garinii cells from complement-mediated lysis suggesting multiple roles of these two variable major proteins in immune evasion of B. miyamotoi.
Implementing an automated monitoring process in a digital, longitudinal observational cohort study

Background: Clinical data collection requires correct and complete data sets in order to perform correct statistical analysis and draw valid conclusions. While in randomized clinical trials much effort concentrates on data monitoring, this is rarely the case in observational studies- due to high numbers of cases and often-restricted resources. We have developed a valid and cost-effective monitoring tool, which can substantially contribute to an increased data quality in observational research.
Methods: An automated digital monitoring system for cohort studies developed by the German Rheumatism Research Centre (DRFZ) was tested within the disease register RABBIT-SpA, a longitudinal observational study including patients with axial spondyloarthritis and psoriatic arthritis. Physicians and patients complete electronic case report forms (eCRF) twice a year for up to 10 years. Automatic plausibility checks were implemented to verify all data after entry into the eCRF. To identify conflicts that cannot be found by this approach, all possible conflicts were compiled into a catalog. This “conflict catalog” was used to create queries, which are displayed as part of the eCRF. The proportion of queried eCRFs and responses were analyzed by descriptive methods. For the analysis of responses, the type of conflict was assigned to either a single conflict only (affecting individual items) or a conflict that required the entire eCRF to be queried.
Results: Data from 1883 patients was analyzed. A total of n = 3145 eCRFs submitted between baseline (T0) and T3 (12 months) had conflicts (40–64%). Fifty-six to 100% of the queries regarding eCRFs that were completely missing were answered. A mean of 1.4 to 2.4 single conflicts occurred per eCRF, of which 59–69% were answered. The most common missing values were CRP, ESR, Schober’s test, data on systemic glucocorticoid therapy, and presence of enthesitis.
Conclusion: Providing high data quality in large observational cohort studies is a major challenge, which requires careful monitoring. An automated monitoring process was successfully implemented and well accepted by the study centers. Two thirds of the queries were answered with new data. While conventional manual monitoring is resource-intensive and may itself create new sources of errors, automated processes are a convenient way to augment data quality.
Background: Amyloid precursor protein (APP) processing is central to Alzheimer’s disease (AD) etiology. As early cognitive alterations in AD are strongly correlated to abnormal information processing due to increasing synaptic impairment, it is crucial to characterize how peptides generated through APP cleavage modulate synapse function. We previously described a novel APP processing pathway producing η-secretase-derived peptides (Aη) and revealed that Aη–α, the longest form of Aη produced by η-secretase and α-secretase cleavage, impaired hippocampal long-term potentiation (LTP) ex vivo and neuronal activity in vivo.
Methods: With the intention of going beyond this initial observation, we performed a comprehensive analysis to further characterize the effects of both Aη-α and the shorter Aη-β peptide on hippocampus function using ex vivo field electrophysiology, in vivo multiphoton calcium imaging, and in vivo electrophysiology.
Results: We demonstrate that both synthetic peptides acutely impair LTP at low nanomolar concentrations ex vivo and reveal the N-terminus to be a primary site of activity. We further show that Aη-β, like Aη–α, inhibits neuronal activity in vivo and provide confirmation of LTP impairment by Aη–α in vivo.
Conclusions: These results provide novel insights into the functional role of the recently discovered η-secretase-derived products and suggest that Aη peptides represent important, pathophysiologically relevant, modulators of hippocampal network activity, with profound implications for APP-targeting therapeutic strategies in AD.
The duration of infectivity of SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) in living patients has been demarcated. In contrast, a possible SARS-CoV-2 infectivity of corpses and subsequently its duration under post mortem circumstances remain to be elucidated. The aim of this study was to investigate the infectivity and its duration of deceased COVID-19 (coronavirus disease) patients. Four SARS-CoV-2 infected deceased patients were subjected to medicolegal autopsy. Post mortem intervals (PMI) of 1, 4, 9 and 17 days, respectively, were documented. During autopsy, swabs and organ samples were taken and examined by RT-qPCR (real-time reverse transcription-polymerase chain reaction) for the detection of SARS-CoV-2 ribonucleic acid (RNA). Determination of infectivity was performed by means of virus isolation in cell culture. In two cases, virus isolation was successful for swabs and tissue samples of the respiratory tract (PMI 4 and 17 days). The two infectious cases showed a shorter duration of COVID-19 until death than the two non-infectious cases (2 and 11 days, respectively, compared to > 19 days), which correlates with studies of living patients, in which infectivity could be narrowed to about 6 days before to 12 days after symptom onset. Most notably, infectivity was still present in one of the COVID-19 corpses after a post-mortem interval of 17 days and despite already visible signs of decomposition. To prevent SARS-CoV-2 infections in all professional groups involved in the handling and examination of COVID-19 corpses, adequate personal safety standards (reducing or avoiding aerosol formation and wearing FFP3 [filtering face piece class 3] masks) have to be enforced for routine procedures.
The advancement of medical technology has led not only to an increase in life expectancy but also to a rise in aging-related diseases. Aging promotes metabolic disorders, in turn affecting cardiovascular health. Derailment of biological processes in the pancreas, liver, adipose tissue, and skeletal muscle impairs glucose and lipid metabolism, and mitochondrial function, triggering the development of diabetes and lipid-related disorders that inflict damage on cardiac and vascular tissues. Long noncoding RNAs (lncRNAs) regulate a wide range of biological process and are one of the key factors controlling metabolism and mitochondria. Here, we discuss the versatile function of lncRNAs involved in the metabolic regulation of glucose and lipid, and mitochondrial function, and how the dysregulation of lncRNAs induces the development of various metabolic disorders and their cardiovascular consequences.
Since 2010, an intensified ambulatory cardiology care programme has been implemented in southern Germany. To improve patient management, the structure of cardiac disease management was improved, guideline-recommended care was supported, new ambulatory medical services and a morbidity-adapted reimbursement system were set up. Our aim was to determine the effects of this programme on the mortality and hospitalisation of enrolled patients with cardiac disorders. We conducted a comparative observational study in 2015 and 2016, based on insurance claims data. Overall, 13,404 enrolled patients with chronic heart failure (CHF) and 19,537 with coronary artery disease (CAD) were compared, respectively, to 8,776 and 16,696 patients that were receiving usual ambulatory cardiology care. Compared to the control group, patients enrolled in the programme had lower mortality (Hazard Ratio: 0.84; 95% CI: 0.77–0.91) and fewer all-cause hospitalisations (Rate Ratio: 0.94; 95% CI: 0.90–0.97). CHF-related hospitalisations in patients with CHF were also reduced (Rate Ratio: 0.76; 95% CI: 0.69–0.84). CAD patients showed a similar reduction in mortality rates (Hazard Ratio: 0.81; 95% CI: 0.76–0.88) and all-cause hospitalisation (Rate Ratio: 0.94; 95% CI: 0.91–0.97), but there was no effect on CAD-related hospitalisation. We conclude that intensified ambulatory care reduced mortality and hospitalisation in cardiology patients.
Background: To determine whether there is a significant saving of time when using a digital cataract workflow for digital data transfer compared to a manual approach of biometry assessment, data export, intraocular lens calculation, and surgery time.
Methods: In total, 48 eyes of 24 patients were divided into two groups: 24 eyes were evaluated using a manual approach, whereas another 24 eyes underwent a full digital lens surgery workflow. The primary variables for comparison between both groups were the overall time as well as several time steps starting at optical biometry acquisition until the end of the surgical lens implantation. Other outcomes, such as toric intraocular lens misalignment, reduction of cylinder, surgically induced astigmatism, prediction error, and distance visual acuity were measured.
Results: Overall, the total diagnostic and surgical time was reduced from 1364.1 ± 202.6 s in the manual group to 1125.8 ± 183.2 s in the digital group (p < 0.001). The complete time of surgery declined from 756.5 ± 82.3 s to 667.3 ± 56.3 (p < 0.0005). Compared to the manual approach of biometric data export and intraocular lens calculation (76.7 ± 12.3 s) as well as the manual export of the reference image to a portable external storage device (26.8 ± 5.5 s), a highly significant saving of time was achieved (p < 0.0001).
Conclusions: Using a software-based digital approach to toric intraocular lens implantation is convenient, more efficient, and thus more economical than a manual workflow in surgery practice.
Background: Nearly all patients with newly diagnosed glioblastoma experience recurrence following standard-of-care radiotherapy (RT) + temozolomide (TMZ). The purpose of the phase III randomized CheckMate 548 study was to evaluate RT + TMZ combined with the immune checkpoint inhibitor nivolumab (NIVO) or placebo (PBO) in patients with newly diagnosed glioblastoma with methylated MGMT promoter (NCT02667587).
Methods: Patients (N = 716) were randomized 1:1 to NIVO [(240 mg every 2 weeks × 8, then 480 mg every 4 weeks) + RT (60 Gy over 6 weeks) + TMZ (75 mg/m2 once daily during RT, then 150-200 mg/m2 once daily on days 1-5 of every 28-day cycle × 6)] or PBO + RT + TMZ following the same regimen. The primary endpoints were progression-free survival (PFS) and overall survival (OS) in patients without baseline corticosteroids and in all randomized patients.
Results: As of December 22, 2020, median (m)PFS (blinded independent central review) was 10.6 months (95% CI, 8.9-11.8) with NIVO + RT + TMZ vs 10.3 months (95% CI, 9.7-12.5) with PBO + RT + TMZ (HR, 1.1; 95% CI, 0.9-1.3) and mOS was 28.9 months (95% CI, 24.4-31.6) vs 32.1 months (95% CI, 29.4-33.8), respectively (HR, 1.1; 95% CI, 0.9-1.3). In patients without baseline corticosteroids, mOS was 31.3 months (95% CI, 28.6-34.8) with NIVO + RT + TMZ vs 33.0 months (95% CI, 31.0-35.1) with PBO + RT + TMZ (HR, 1.1; 95% CI, 0.9-1.4). Grade 3/4 treatment-related adverse event rates were 52.4% vs 33.6%, respectively.
Conclusions: NIVO added to RT + TMZ did not improve survival in patients with newly diagnosed glioblastoma with methylated or indeterminate MGMT promoter. No new safety signals were observed.
Activated SUMOylation restricts MHC class I antigen presentation to confer immune evasion in cancer

Activated SUMOylation is a hallmark of cancer. Starting from a targeted screening for SUMO-regulated immune evasion mechanisms, we identified an evolutionarily conserved function of activated SUMOylation, which attenuated the immunogenicity of tumor cells. Activated SUMOylation allowed cancer cells to evade CD8+ T cell–mediated immunosurveillance by suppressing the MHC class I (MHC-I) antigen-processing and presentation machinery (APM). Loss of the MHC-I APM is a frequent cause of resistance to cancer immunotherapies, and the pharmacological inhibition of SUMOylation (SUMOi) resulted in reduced activity of the transcriptional repressor scaffold attachment factor B (SAFB) and induction of the MHC-I APM. Consequently, SUMOi enhanced the presentation of antigens and the susceptibility of tumor cells to CD8+ T cell–mediated killing. Importantly, SUMOi also triggered the activation of CD8+ T cells and thereby drove a feed-forward loop amplifying the specific antitumor immune response. In summary, we showed that activated SUMOylation allowed tumor cells to evade antitumor immunosurveillance, and we have expanded the understanding of SUMOi as a rational therapeutic strategy for enhancing the efficacy of cancer immunotherapies.
Purpose: To investigate the diagnostic performance of noise-optimized virtual monoenergetic images (VMI+) in dual-energy CT (DECT) of portal vein thrombosis (PVT) compared to standard reconstructions. Method: This retrospective, single-center study included 107 patients (68 men; mean age, 60.1 ± 10.7 years) with malignant or cirrhotic liver disease and suspected PVT who had undergone contrast-enhanced portal-phase DECT of the abdomen. Linearly blended (M_0.6) and virtual monoenergetic images were calculated using both standard VMI and noise-optimized VMI+ algorithms in 20 keV increments from 40 to 100 keV. Quantitative measurements were performed in the portal vein for objective contrast-to-noise ratio (CNR) calculation. The image series showing the greatest CNR were further assessed for subjective image quality and diagnostic accuracy of PVT detection by two blinded radiologists. Results: PVT was present in 38 subjects. VMI+ reconstructions at 40 keV revealed the best objective image quality (CNR, 9.6 ± 4.3) compared to all other image reconstructions (p < 0.01). In the standard VMI series, CNR peaked at 60 keV (CNR, 4.7 ± 2.1). Qualitative image parameters showed the highest image quality rating scores for the 60 keV VMI+ series (median, 4) (p ≤ 0.03). The greatest diagnostic accuracy for the diagnosis of PVT was found for the 40 keV VMI+ series (sensitivity, 96%; specificity, 96%) compared to M_0.6 images (sensitivity, 87%; specificity, 92%), 60 keV VMI (sensitivity, 87%; specificity, 97%), and 60 keV VMI+ reconstructions (sensitivity, 92%; specificity, 97%) (p ≤ 0.01). Conclusions: Low-keV VMI+ reconstructions resulted in significantly improved diagnostic performance for the detection of PVT compared to other DECT reconstruction algorithms.
Mesenchymal stromal/stem cells and their derivates are the most promising cell source for cell therapies in regenerative medicine. The application of extracellular vesicles (EVs) as cell-free therapeuticals requires particles with a maximum regenerative capability to enhance tissue and organ regeneration. The cargo of mRNA and microRNA (miR) in EVs after hypoxic preconditioning has not been extensively investigated. Therefore, the aim of our study was the characterization of mRNA and the miR loading of EVs. We further investigated the effects of the isolated EVs on renal tubular epithelial cells in vitro. We found 3131 transcripts to be significantly regulated upon hypoxia. Only 15 of these were downregulated, but 3116 were up-regulated. In addition, we found 190 small RNAs, 169 of these were miRs and 21 were piwi-interacting RNAs (piR). However, only 18 of the small RNAs were significantly altered, seven were miRs and 11 were piRs. Interestingly, all seven miRs were down-regulated after hypoxic pretreatment, whereas all 11 piRs were up-regulated. Gene ontology term enrichment and miR-target enrichment analysis of the mRNAs and miR were also performed in order to study the biological background. Finally, the therapeutic effect of EVs on human renal tubular epithelial cells was shown by the increased expression of three anti-inflammatory molecules after incubation with EVs from hypoxic pretreatment. In summary, our study demonstrates the altered mRNA and miR load in EVs after hypoxic preconditioning, and their anti-inflammatory effect on epithelial cells.
Whereas the clinical approach in pediatric cancer patients with febrile neutropenia is well established, data on non-neutropenic infectious episodes are limited. We therefore prospectively collected over a period of 4 years of data on all infectious complications in children treated for acute lymphoblastic or myeloid leukemia (ALL or AML) and non-Hodgkin lymphoma (NHL) at two major pediatric cancer centers. Infections were categorized as fever of unknown origin (FUO), and microbiologically or clinically documented infections. A total of 210 patients (median age 6 years; 142 ALL, 23 AML, 38 NHL, 7 leukemia relapse) experienced a total of 776 infectious episodes (571 during neutropenia, 205 without neutropenia). The distribution of FUO, microbiologically and clinically documented infections, did not significantly differ between neutropenic and non-neutropenic episodes. In contrast to neutropenic patients, corticosteroids did not have an impact on the infectious risk in non-neutropenic children. All but one bloodstream infection in non-neutropenic patients were due to Gram-positive pathogens. Three patients died in the context of non-neutropenic infectious episodes (mortality 1.4%). Our results well help to inform clinical practice guidelines in pediatric non-neutropenic cancer patients presenting with fever, in their attempt to safely restrict broad-spectrum antibiotics and improve the quality of life by decreasing hospitalization.
Bei fehlender Milz nach einer totalen Splenektomie kann es unter anderem zum lebensgefährlichen Postsplenektomie-Syndrom (overwhelming postplenectomy infection, OPSI) kommen, weshalb die partielle Splenektomie (PS) zunehmend in den Fokus rückt. Die Reduktion perioperativer Komplikationen erscheint zudem wünschenswert, weswegen der laparoskopische Operationsansatz zunehmende medizinische Resonanz genießt. Letzteren verbinden Chirurgen häufig mit geringeren postoperativen Schmerzen für den Patienten, besseren kosmetischen Ergebnissen und einer Reduktion chirurgischer Komplikationen.
Ziel dieser Studie war es, anhand verschiedener Parameter die perioperativen Ergebnisse zwischen der laparoskopischen partiellen Splenektomie (LPS) und der offenen partiellen Splenektomie (OPS) bei Kindern und Jugendlichen zu vergleichen. Betrachtet wurde ein Zeitraum von etwa zehn Jahren (2008-2018), wobei insgesamt 26 Patienten die Einschlusskriterien erfüllten. 10 Patienten unterzogen sich der LPS, während bei 16 Patienten eine OPS durchgeführt wurde. Anhand der digitalen Patientenakten wurden verschiedene perioperative Parameter erhoben, welche unter anderem die postoperative Schmerzstärke, den Analgetikabedarf, verschiedene hämatologische Variablen sowie die perioperativen Komplikationen umfassten.
Zwischen den beiden Gruppen unterschieden sich das postoperative Schmerzbefinden mit entsprechend analgetischer Therapie, der zeitliche Beginn des oralen Kostaufbaus und der postoperativen Mobilisation sowie die Dauer des stationären Aufenthalts nicht signifikant. Ferner wurden die chirurgischen Komplikationen anhand der Clavien-Dindo-Klassifikation (CDC) in verschiedene Grade eingeteilt und konnten sowohl in der laparoskopischen als auch in der offen-chirurgisch operierten Gruppe zeigen, dass hauptsächlich leichte Komplikationen vorlagen, bei denen ein Großteil der Patienten lediglich eine medikamentöse Therapie (80.8%) für die aufgetretenen Komplikationen (CDC ≤ II) benötigte. Lediglich bei 13% der offen operierten Patienten bedurfte es einer chirurgischen bzw. endoskopischen Intervention (CDC ≥ III).
Die Operationsdauer zwischen den beiden Gruppen unterschied sich jedoch signifikant. Bei der partiellen Splenektomie nahm der laparoskopische Zugangsweg signifikant mehr Zeit in Anspruch als bei der offen-chirurgisch operierten Gruppe. Darüber hinaus wurde der perioperative Blutverlust anhand der Mercuriali-Formel berechnet und zeigte bei der LPS-Gruppe einen signifikant höheren perioperativen Blutverlust als bei der OPS.
Die durch diese Studie erzielten Resultate zeigen anhand verschiedener perioperativer Parameter, dass die LPS im Vergleich zur etablierten OPS trotz signifikanter Unterschiede in der Operationszeit und dem perioperativen Blutverlust beide sichere und praktikable Operationsverfahren sind. Zusammenfassend stellten sich hinsichtlich der weiteren perioperativen Ergebnisse keine signifikanten Unterschiede zwischen beiden Gruppen dar.
Im Rahmen unserer Untersuchung konnte zudem dargestellt werden, dass die postoperativen Rekonvaleszenzvariablen sowie die Scores zur Erfassung der chirurgischen Komplikationen keinen signifikanten Unterschied ergaben. Ebenso wies die postoperativ dokumentierte Schmerzstärke der Patienten und der daraus resultierende Analgetikabedarf keine signifikanten Unterschiede auf.
Der Forschung von Kardiologen wie etwa Joseph C. Wu, Professor an der kalifornischen Stanford-Universität, ist es zu verdanken, dass die Sterblichkeit nach einem Herzinfarkt von ehemals 15 Prozent auf nur noch 2 bis 3 Prozent gesunken ist. Auf Einladung der Friedrich-Merz-Stiftungsgastprofessur besuchte Wu, Präsident der »American Heart Association« und derzeit einer der prominentesten Herz-Forscher, für eine Woche die Goethe-Universität, um sich sowohl mit Forschenden als auch mit Studierenden auszutauschen. In einem Bürgergespräch stellte er sich außerdem den Fragen der Frankfurter Bevölkerung, unter anderem zusammen mit Stefanie Dimmeler, Sprecherin des »Deutschen Zentrums für Herz-Kreislauf-Erkrankungen« und Leiterin des Exzellenzclusters »Cardio-Pulmonary Institute«.
Purpose: We evaluated efficacy and safety profile of patients with anticoagulation therapy (AT) undergoing holmium laser enucleation of the prostate (HoLEP).
Methods: Within our prospective institutional database (11/2017 to 11/2019), we analyzed functional outcomes and 30-day complication rates of HoLEP patients according to Clavien–Dindo classification (CLD), stratified according to specific AT vs. no AT. Further analyses consisted of uni- and multivariate logistic regression models (LRM) predicting complications.
Results: Of 268 patients undergoing HoLEP, 104 (38.8%) received AT: 25.7% were treated with platelet aggregation inhibitors (PAI), 8.2% with new oral anticoagulants (NOAC) and 4.9% with AT-combinations or coumarins bridged with low molecular weight heparins (LMWH/combination). Patients receiving AT were significantly more comorbid (p < 0.01). Pre- and postoperative maximal flow rates, residual void urine and IPSS at 3 months after surgery were invariably improved after HoLEP for patients with/ without AT. Overall complication rate was 19.5% in patients with no AT vs. 26.1% vs. 27.3 vs. 46.2%, respectively, in patients with PAI, NOAC and LMWH/combination (p < 0.01). Major complications (CLD ≥ 3b) occurred in 6.1% of no AT patients vs. 4.3% vs. 4.5 vs. 0% in patients with PAI, NOAC and LMWH/combination, respectively (p < 0.01). In multivariate LRM, AT was not significantly associated with higher complication rates, whereas high ASA status (OR 2.2, p = 0.04), age (OR 1.04, p = 0.02) and bioptical or incidental prostate cancer (OR 2.5, p = 0.01) represented independent risk factors.
Conclusion: Despite higher overall complication rates in AT patients, major complications were not more frequent in AT patients. HoLEP is safe and effective in anticoagulated patients.
Zinc finger proteins (ZNF) are a large group of transcription factors with diverse functions. We recently discovered that endothelial cells harbour a specific mechanism to limit the action of ZNF354C, whose function in endothelial cells is unknown. Given that ZNF354C has so far only been studied in bone and tumour, its function was determined in endothelial cells. ZNF354C is expressed in vascular cells and localises to the nucleus and cytoplasm. Overexpression of ZNF354C in human endothelial cells results in a marked inhibition of endothelial sprouting. RNA-sequencing of human microvascular endothelial cells with and without overexpression of ZNF354C revealed that the protein is a potent transcriptional repressor. ZNF354C contains an active KRAB domain which mediates this suppression as shown by mutagenesis analysis. ZNF354C interacts with dsDNA, TRIM28 and histones, as observed by proximity ligation and immunoprecipitation. Moreover, chromatin immunoprecipitation revealed that the ZNF binds to specific endothelial-relevant target-gene promoters. ZNF354C suppresses these genes as shown by CRISPR/Cas knockout and RNAi. Inhibition of endothelial sprouting by ZNF354C is dependent on the amino acids DV and MLE of the KRAB domain. These results demonstrate that ZNF354C is a repressive transcription factor which acts through a KRAB domain to inhibit endothelial angiogenic sprouting.
Aim: Assessment of the effect of nonsurgical periodontal therapy on haematological parameters in patients with grades B (BP) and C periodontitis (CP).
Methods: Eight BP and 46 CP patients received full-mouth periodontal debridement within 48 h, if positive for Aggregatibacter actinomycetemcomitans with adjunctive systemic antibiotics (4 BP, 17 CP). Clinical data were collected prior and 12 weeks after periodontal therapy. Blood was sampled prior to and 1 day as well as 6 and 12 weeks after the first SD visit. Erythrocyte count, haemoglobin value, haematocrit (HCT), mean erythrocyte volume (MCV), mean corpuscular haemoglobin (MCH), MCH concentration (MCHC), platelets (PLT) and heat shock protein 27 (Hsp27) were assessed.
Results: Both groups showed significant clinical improvement (p < 0.05). Using univariate analysis, MCV was noticeably lower in CP than BP at all examinations, HCT only at baseline. For CP, MCHC was noticeably higher 12 weeks after SD than at baseline and 1 day (p ≤ 0.005) and Hsp27 increased noticeably at 1 day (p < 0.05). Repeated measures analysis of variance revealed African origin to be associated with lower MCV and female sex with lower MCHC.
Conclusion: Based on multivariate analysis, periodontal diagnosis (BP/CP) was not associated with haematological parameters measured in this study or serum Hsp27. In CP, nonsurgical periodontal therapy improved MCHC 12 weeks after SD. Also in CP Hsp27 was increased 1 day after SD.
Matrix metalloproteinases (MMPs) play crucial roles in tissue homeostasis and pathologies by remodeling the extracellular matrix. Previous studies have demonstrated the biological activities of MMP-derived cleavage products. Furthermore, specific fragments can serve as biomarkers. Therefore, an in vitro cleavage assay to identify substrates and characterize cleavage patterns could provide important insight in disease-relevant mechanisms and the identification of novel biomarkers. In the pathogenesis of osteoarthritis (OA), MMP-2, -8, -9 and -13 are of vital importance. However, it is unclear which protease can cleave which matrix component. To address this question, we established an in vitro cleavage assay using recombinantly expressed MMPs and the two cartilage matrix components, COMP and thrombospondin-4. We found a time- and concentration-dependent degradation and an MMP-specific cleavage pattern for both proteins. Cleavage products can now be enriched and purified to investigate their biological activity. To verify the in vivo relevance, we compared the in vitro cleavage patterns with serum and synovial fluid from OA patients and could indeed detect fragments of similar size in the human samples. The cleavage assay can be adapted to other MMPs and substrates, making it a valuable tool for many research fields.
Different treatment options for acetabular fractures in the elderly and nonagenarians exist; a consistent guideline has not been established, yet. The purpose of this study is to give an overview of how those fractures can be handled and compares two different surgical treatment methods.
A total of 89 patients ≥ 18 years between 2016 and 2021 with acetabular fractures in our department received a surgical intervention with plate fixation via the Stoppa approach or a total hip arthroplasty with a Burch–Schneider ring and integrated cup. 60 patients ≥ 65 were compared in two groups, 29 patients between 65 and 79 and 31 patients ≥ 80. For comparison, data on operation times, hospitalization, complications during operation and hospital stay, blood loss and postoperative mobilization were collected.
Characteristics could be found for indications for operative osteosynthesis or endoprosthetics based on the X-ray analysis. There was a tendency to treat simple fractures with osteosynthesis. Patients between 65 and 79 with an osteosynthesis had benefits in almost every comparison. Patients ≥ 80 with a plate fixation had advantages in the categories of postoperative complications, blood loss and transfusion of erythrocyte concentrates. Statistical significant differences were noticed in both groups regarding the operation time. Patients between 65 and 79 with osteosynthesis had significant benefits for postoperative complications, hospitalization, number of blood transfusions and postoperative mobilization.
Finding the best supportive treatment option is difficult, and decision-making must respect fracture patterns and individual risk factors. This study shows that plate fixation via the Stoppa approach has some benefits.
Ziel unserer retrospektiven Studie war es, die refraktiven und kornealen Veränderungen nach DMEK bei pseudophaken Patienten, die sich auf Grund einer endothelialen Hornhauterkrankung behandeln ließen, zu untersuchen. Durch unsere einheitlich pseudophake Patientenkohorte wollten wir untersuchen, ob sich die refraktiven Veränderungen nach DMEK von den bereits bekannten Änderungen bei einer simultan durchgeführten Katarakt- und DMEK-Operation sogenannte „Triple“-DMEK unterscheiden. Primärer Endpunkt der Studie war die Veränderung der Refraktion unter besonderer Berücksichtigung des sphärischen Äquivalents (SEQ) des jeweiligen pseudophaken Auges nach DMEK. Sekundäre Endpunkte umfassten die Entwicklung des Visus, der CCT, der ECD und verschiedener kornealer Parameter, die mittels Scheimpflug- Tomographie ermittelt wurden.
In der vorliegenden Arbeit erfolgte hierzu die retrospektive Analyse von Daten, die in den Patientenakten dokumentiert und digital gespeichert waren (Pentacam® HR). Es wurden 109 Augen von 95 Patienten, die sich im Zeitraum von Februar 2015 bis Dezember 2018 mittels DMEK in unserem Zentrum behandeln ließen, in die Studie eingeschlossen. Davon stammten 66 Augen (61%) von weiblichen und 43 Augen (39%) von männlichen Patienten. Es handelte sich bei 61 Augen (56%) um ein linkes und bei 48 Augen (44 %) um ein rechtes Auge. Die Patienten waren 20 bis 91 Jahre alt. Das mittlere Alter zum Zeitpunkt der DMEK-Operation betrug 71,9 Jahre (SD ±10,23). Der Altersmittelwert der Männer lag bei 70,4 Jahren (SD 11,23, Spannweite: 20-84) und der der Frauen bei 72,9 Jahren (SD 9,49, Spannweite: 47-91). Der mittlere Nachbeobachtungszeitraum betrug 10,47 Monate (SD 6,78, Spannweite: 1-28 Monate).
Für das SEQ konnte bei Betrachtung aller ausgewerteten Daten eine leichte Tendenz in Richtung eines „hyperopen shifts“ mit einer mittleren Veränderung des SEQ von + 0,1 D gezeigt werden, die jedoch nicht statistisch signifikant war. Zwischenzeitlich kam es im Nachbeobachtungszeitraum zu einer klaren Tendenz hinsichtlich einer Myopisierung bei Betrachtung aller ausgewerteten Daten. Der „hyperope shift“ konnte erst am Ende, möglicherweise infolge einer deutlich reduzierten Patientenzahl, beobachtet werden. In der Subgruppe „vollständige Kontrollen“ für das SEQ zeigte sich eine Tendenz hinsichtlich einer leichten Abnahme des SEQ, die jedoch ebenfalls nicht statistisch signifikant war und aufgrund der geringen Gruppengröße (n=32) kritisch betrachtet werden sollte. In unserer Kohorte konnte somit keine eindeutige Aussage über eine postoperative Änderung des SEQ in Richtung eines „hyperopen shifts“ oder „myopen shifts“ gemacht werden. Die Refraktion verhielt sich in unserem Patientenkollektiv nach DMEK insgesamt weitestgehend stabil. Insofern sind refraktive "Überraschungen", wie sie weiterhin häufig nach "Triple"-DMEK zu beobachten sind, bei zuvor pseudophakisierten Patienten in einem weitaus geringeren Maße zu erwarten. Unter den mittels Scheimpflug-Technologie untersuchten kornealen Parametern wies lediglich der posteriore Astigmatismus signifikante Veränderungen im Sinne einer Reduktion der kornealen Krümmung auf. Indirekt heben unsere Ergebnisse damit die Bedeutung des posterioren Hornhautprofils auf die postoperative Refraktionsentwicklung und somit auch auf die IOL-Kalkulation bei "Triple"-DMEK-Prozeduren hervor. Außerdem scheint der postoperative Anstieg der Sehschärfe mit den gleichfalls signifikanten Änderungen der kornealen Densitometrie in der 2-6 mm Zone, des Hornhautvolumens und der zentralen Hornhautdicke umgekehrt korreliert zu sein.
Darüber hinaus lässt sich festhalten, dass auch unsere Untersuchung in einem gewissen Maße die Überlegenheit der DMEK gegenüber etablierten Techniken, wie beispielsweise der PK verdeutlicht. Sowohl anhand der refraktiven Stabilität als auch der Visusergebnisse konnten wir belegen, dass das DMEK-Verfahren nach der anfänglichen Lernkurve die wohl besten funktionellen Ergebnisse in der Behandlung von Patienten mit endothelialen Hornhauterkrankungen liefert. Besonders bei bereits pseudophaken Patienten weist die DMEK durch die zu erwartende hohe postoperative refraktive Stabilität viele Vorteile auf
und erscheint insbesondere hinsichtlich der Vorhersagbarkeit des postoperativen refraktiven Ergebnisses der „Triple“- DMEK überlegen.
Acute kidney injury (AKI) is still associated with high morbidity and mortality incidence rates, and also bears an elevated risk of subsequent chronic kidney disease. Although the kidney has a remarkable capacity for regeneration after injury and may recover completely depending on the type of renal lesions, the options for clinical intervention are restricted to fluid management and extracorporeal kidney support. The development of novel therapies to prevent AKI, to improve renal regeneration capacity after AKI, and to preserve renal function is urgently needed. The Special Issue covers research articles that investigated the molecular mechanisms of inflammation and injury during different renal pathologies, renal regeneration, diagnostics using new biomarkers, and the effects of different stimuli like medication or bacterial components on isolated renal cells or in vivo models. The Special Issue contains important reviews that consider the current knowledge of cell death and regeneration, inflammation, and the molecular mechanisms of kidney diseases. In addition, the potential of cell-based therapy approaches that use mesenchymal stromal/stem cells or their derivates is summarized. This edition is complemented by reviews that deal with the current data situation on other specific topics like diabetes and diabetic nephropathy or new therapeutic targets.
Untersuchung von long non-coding RNA im Entzündungsmodell mit mesenchymalen Stamm-/Stromazellen

Entzündungsprozesse sind essentiell zur Abwehr exogener und endogener Pathogene sowie bei der Geweberegeneration. Ihre Dysregulation ist an unzähligen Krankheitsprozessen beteiligt. Die Auslösung einer Entzündung ist besonders gut untersucht bei Toll-like Rezeptoren, die Strukturen von Mikroorganismen erkennen können und durch Signalkaskaden beispielsweise ΝF-κB aktivieren. LncRNAs regulieren die Genexpression und wurden dabei bereits im Rahmen von Entwicklung, Proliferation, Karzinogenese und Entzündung nachgewiesen. ASC sind für die regenerative Medizin aufgrund ihrer einfachen Gewinnung und ihrer Differenzierbarkeit höchst interessant. Zudem haben sie einen Einfluss auf inflammatorische Prozesse. Daher könnte es relevant sein, welche Rolle lncRNAs während Entzündungsprozessen bei ASC spielen. Daraus könnten sich auch potentielle Ansätze für Diagnostik und Therapie entwickeln.
Es wurde ein Entzündungsmodell mit ASC etabliert, welche mit Bakterientoxinen stimuliert wurden. Das Modell wurde mit einem im nephrologischen Labor etablierten Modell mit renalen Epithelzellen hinsichtlich der Entzündungsantwort verglichen. Diese Entzündungsantwort wurde anhand der Zytokinproduktion auf mRNA- und Proteinebene quantifiziert. Anschließend erfolgte eine RNA-Sequenzierung und Vergleich der RNA bei stimulierten und nicht-stimulierten ASC. Die detektierten veränderten lncRNAs wurden mittels qPCR validiert. Zuletzt wurde durch knockdown einer ausgewählten lncRNA versucht, Einfluss auf die Entzündungsprozesse zu nehmen.
Die vorgelegte Arbeit zeigt deutlich, dass ASC eine stärkere Entzündungsantwort als renale Epithelzellen zeigen. Als Mittelweg aus maximaler Entzündungsantwort und realistischen Stimulationsbedingungen wurde eine Stimulation mit 10 ng/ml LPS für 4 h gewählt. Nach der RNA-Sequenzierung zeigte die funktionelle Analyse der veränderten codierenden RNA Hinweise auf Entzündungsprozesse, Zellmigration, Chemotaxis, Differenzierung, Proliferation sowie Alkoholismus, Atherosklerose, Diabetes mellitus und Insulinresistenz. Diese Ergebnisse lassen sich mit dem aktuellen Stand der Forschung in Einklang bringen und legen die Bedeutung von Entzündung und ASC in diesen Bereichen dar. Bei den lncRNAs ergab die Sequenzierung insgesamt 48 expressionsveränderte Transkripte, H19 konnte hierbei erfolgreich validiert werden. Diese lncRNA war im Rahmen der LPS-induzierten Entzündung expressionsvermindert. Aufgrund donorspezifischer Einflussfaktoren auf die Genexpression bei ASC wie Körpergewicht und Morbidität sowie allgemeiner interindividueller Schwankung der Expression von lncRNA sind aber weitere Untersuchungen zur Detektion relevanter lncRNAs erforderlich. Die Transfektionsversuche zeigten Hinweise darauf, dass der H19-knockdown möglicherweise die LPS-vermittelte Entzündungsantwort bei ASC verstärkt.
Zusammenfassend wurde ein Modell zur Untersuchung von lncRNA bei LPS-induzierter Inflammation in ASC etabliert sowie im Rahmen dessen H19 als relevante lncRNA detektiert, die den Ausgangspunkt für weitere Forschung darstellt.
Acute lymphoblastic leukemia (ALL) is a malignancy of lymphoid progenitor cells occurring at an annual incidence rate of approximately 1.1 to 2.1 per 100,000 person-years globally. Approximately 40% of annual ALL cases occur in adults, yet estimated 5-year overall survival rates are about 40% to 50% in adults (and vary broadly by age) compared with 90% in children. Although the addition and/or intensification of asparaginase as a key treatment strategy for pediatric ALL is well recognized, further research is needed to clarify the benefit/risk ratio in adult patients with ALL. This review emphasizes the importance of efficient management of adverse events to increase asparaginase efficacy and explores novel strategies for optimizing asparaginase treatment, including new formulations of asparaginase, pharmacokinetic-based dosing, and pharmacogenetic profiling. Upcoming results of adult ALL trials should further clarify the role of asparaginase, building on the results of the large NOPHO 2008, CALGB 10403, GRAALL-2005, GMALL 07/2003, and UKALL14 trials.
Autophagy is the highly conserved catabolic process, which enables the survival of a cell under unfavorable environmental conditions. In a constantly changing environment, cells must be capable of dynamically oscillating between anabolism and catabolism in order to maintain cellular homeostasis. In this context, the activity of the mechanistic Target Of Rapamycin Complex 1 (mTORC1) is of major importance. As a central signaling node, it directly controls the process of macroautophagy and thus cellular metabolism. Thereby, the control of mTORC1 is equally crucial as the regulation of cellular homeostasis itself, whereby particular importance is attributed to amino acid sensory proteins. In this review, we describe the recent findings of macroautophagy and mTORC1 regulation by upstream amino acid stimuli in different subcellular localizations. We highlight in detail which proteins of the sensor complexes play a specific role in this regulation and point out additional non-canonical functions, e.g. in the regulation of macroautophagy, which have received little attention so far.
Systematic protein localization and protein-protein interaction studies to characterize specific protein functions are most effectively performed using tag-based assays. Ideally, protein tags are introduced into a gene of interest by homologous recombination to ensure expression from endogenous control elements. However, inefficient homologous recombination makes this approach difficult in mammalian cells. Although gene targeting efficiency by homologous recombination increased dramatically with the development of designer endonuclease systems such as CRISPR/Cas9 capable of inducing DNA double-strand breaks with unprecedented accuracy, the strategies still require synthesis or cloning of homology templates for every single gene. Recent developments have shown that endogenous protein tagging can be achieved efficiently in a homology independent manner. Hence, combinations between CRISPR/Cas9 and generic tag-donor plasmids have been used successfully for targeted gene modifications in mammalian cells. Here, we developed a tool kit comprising a CRISPR/Cas9 expression vector with several EGFP encoding plasmids that should enable tagging of almost every protein expressed in mammalian cells. By performing protein-protein interaction and subcellular localization studies of mTORC1 signal transduction pathway-related proteins expressed in HEK293T cells, we show that tagged proteins faithfully reflect the behavior of their native counterparts under physiological conditions.
Erhöht die Gabe intravenöser Tranexamsäure die Wahrscheinlichkeit für thromboembolische Ereignisse?

Hintergrund: Tranexamsäure (TXA) ist ein Antifibrinolytikum, welches Blutungen effizient reduzieren kann. Auf Grund des Wirkmechanismus bestehen jedoch Bedenken, dass TXA zu einem erhöhten Risiko für thromboembolische Ereignisse (TE) führen könnte.
Zielsetzung: Ziel dieser Arbeit ist die Untersuchung eines möglichen Zusammenhanges zwischen der Applikation intravenöser (iv) TXA und dem Auftreten von TE sowie der Mortalität. Ebenfalls soll ein möglicher Dosis-abhängiger Effekt untersucht werden.
Methoden: Es erfolgte eine systematische Suche der MEDLINE Datenbank und des Cochrane Central Register of Controlled Trials. Berücksichtigt wurden alle randomisiert kontrollierten Studien bis inklusive 2020, welche iv TXA mit Placebo oder einer Kontrollgruppe ohne Intervention verglichen. Die eingeschlossenen Studien sind in englischer, deutscher, spanischer oder französischer Sprache publiziert. Übergeordnete Endpunkte waren die Summe aller TE sowie die
Gesamtmortalität. Zusätzlich wurden die Endpunkte venöse Thrombosen, Lungenembolien, venöse thromboembolische Ereignisse (VTE), Myokardinfarkte, Schlaganfälle oder transitorische ischämische Attacken, Mesenterialischämien, arterielle Verschlüsse, blutungsassoziierte Mortalität sowie nicht-blutungsassoziierte Mortalität untersucht. Anhand der „Preferred Reporting Items for Systematic Reviews and Meta-Analyses“ (PRISMA) wurden die vorliegende Metaanalyse, Subgruppen- und Sensitivitätsanalyse erstellt. Als Effektstärkemaß wurde die Risikodifferenz (RD) berechnet. Für den primären Endpunkt der Summe aller TE erfolgten zusätzliche Sensitivitätsanalysen zur Berechnung des Risikoquotienten (RR). Eine Metaregressionsanalyse wurde zur Untersuchung eines dosisabhängigen Effektes durchgeführt. Das Bias-Risiko der eingeschlossenen Studien wurde anhand des Cochrane Risk of Bias Tool bewertet.

Insgesamt wurden 216 Studien in die vorliegende Metaanalyse eingeschlossen. Die Summe aller TE betrug 1020 (2,1%) in der TXA-Gruppe und 900 (2,0%) in der Kontrollgruppe. Es fand sich kein Zusammenhang zwischen iv TXA und dem Risiko für die Summe aller TE (RD = 0,001; 95%-Konfidenzintervall (CI): -0,001 bis 0,002; P = 0,49) sowie für venöse Thrombosen, Lungenembolien, VTE, Myokardinfarkte oder -ischämien und Schlaganfälle oder transitorische ischämische Attacken. Die Sensitivitätsanalyse für die Summe aller TE zur Berechnung des RR zeigte keine Assoziation mit iv TXA, weder unter Ausschlussder Studien ohne TE (RR = 1,03; 95%CI: 0,95 bis 1,12; P = 0,52) noch unter Einschluss dieser Studien (RR = 1,02; 95%CI: 0,94 bis 1,11; P = 0,56). Die Sensitivitätsanalyse der Studien mit einem geringen Selektionsbias zeigte ein vergleichbares Ergebnis. Die Sensitivitätsanalyse mit Patienten mit einem erhöhten Thromboembolie-Risiko fand keine Assoziation zwischen iv TXA und TE (RD = 0,000; 95%CI: -0,008 bis 0,009; P = 0,95). Die Subgruppenanalyse von Studien mit bis zu 99 Patienten, 100 bis 999 Patienten und 1.000 oder mehr Patienten zeigte keine Assoziation zwischen iv TXA und der Summe aller TE. Die Gabe von iv TXA war mit einer signifikanten Reduktion der Gesamtmortalität (RD = -0,007; 95%CI: -0,012 bis -0,004; P < 0,001) und der blutungsassoziierten Mortalität verbunden. Für die nicht-blutungsassoziierte Mortalität zeigte sich kein signifikanter Zusammenhang. Eine Metaregression mit 143 Interventionsgruppen fand keinen Zusammenhang zwischen der TXA-Dosierung und dem Risiko für VTE.
Diskussion: Die vorliegende Arbeit konnte zeigen, dass keine Assoziation zwischen iv TXA und TE besteht. Die Sensitivitätsanalysen konnten dieses Ergebnis bestätigen. Die Metaregressionsanalyse fand keinen dosisabhängigen Zusammenhang zwischen iv TXA und VTE. Gleichzeitig wird die Gesamtmortalität durch TXA signifikant reduziert. Die vorliegende Analyse unterstützt die sichere Anwendung von iv TXA und legt einen wahrscheinlichen Überlebensvorteil nahe. Die Subgruppenanalysen der neurologischen Patienten lieferten uneindeutige Ergebnisse, weshalb der Nutzen von iv TXA für dieses Patientenkollektiv unklar bleibt.
Purpose: Every physician must be able to sufficiently master medical emergencies, especially in medical areas where emergencies occur frequently such as in the emergency room or emergency surgery. This contrasts with the observation that medical students and young residents often feel insufficiently prepared to handle medical emergencies. It is therefore necessary to train them in the treatment of emergency patients. The aim of this study is to analyze the influence of the assignment of manikin versus simulated patients during a training for undergraduate medical students on learning outcomes and the perceived realism.
Methods: The study had a prospective cross-over design and took place in a 3-day emergency medicine training for undergraduate medical students. Students completed three teaching units (‘chest pain’, ‘impaired consciousness’, ‘dyspnea’), either with manikin or simulated patient. Using a questionnaire after each unit, overall impression, didactics, content, the quality of practical exercises, and the learning success were evaluated. The gained competences were measured in a 6-station objective structured clinical examination (OSCE) at the end of training.
Results: 126 students participated. Students rated simulated patients as significantly more realistic than manikins regarding the possibility to carry out examination techniques and taking medical history. 54.92% of the students would prefer to train with simulated patients in the future. Regarding the gained competences for ‘chest pain’ and ‘impaired consciousness’, students who trained with a manikin scored less in the OSCE station than the simulated patients-group.
Conclusion: Simulated patients are rated more realistic than manikins and seem to be superior to manikins regarding gained competence.
Background: Blood pressure is known to be increased in kidney donors following living-donor kidney transplantation. However, the physiological underpinnings of the blood-pressure increase following uninephrectomy remain unclear. We hypothesized that changes in sympathetic tone or in parasympathetic modulation of sinus node function are involved in the blood-pressure increase following experimental kidney-mass reduction.
Methods: C57BL6N mice (6 to 11 per group) subjected to sham surgery (controls) or uninephrectomy with or without a one-week course of sodium chloride-enriched, taurine-deficient diet were studied. Uninephrectomized mice treated with a subcutaneous infusion of angiotensin-II over a period of one week were positive controls. A transfemoral aortic catheter with telemetry unit was implanted, readings of heart-rate and blood-pressure were recorded. Powerspectral analysis of heart rate and systolic blood pressure was performed to gain surrogate parameters of sympathetictone and parasympathetic modulation of sinus node function. Baroreflex sensitivity of heart rate was determined from awake, unrestrained mice using spontaneous baroreflex gain technique.
Results: Systolic arterial blood pressure, heart rate and baroreflex sensitivity were not different in uninephrectomized mice when compared to controls. Parasympathetic modulation of sinus node function was less in uninephrectomized mice in comparison to controls. Uninephrectomized mice of the high-angiotensin-II model or of the high-salt and taurine-deficiency model had an increased systolic arterial blood pressure.
Conclusions: Uninephrectomy associated with less parasympathetic modulation of sinus node function. The combination of uninephrectomy, taurine-deficiency and high-salt intake led to arterial hypertension.
Background & Aims: Liver fibrosis arises from long-term chronic liver injury, accompanied by an accelerated wound healing response with interstitial accumulation of extracellular matrix (ECM). Activated hepatic stellate cells (HSC) are the main source for ECM production. MicroRNA29a (miR-29a) is a crucial antifibrotic miRNA that is repressed during fibrosis, resulting in up-regulation of collagen synthesis.
Methods; Intracellular and extracellular miRNA levels of primary and immortalized myofibroblastic HSC in response to profibrogenic stimulation by transforming growth factor β (TGFβ) or platelet-derived growth factor-BB (PDGF-BB) or upon inhibition of vesicular transport and autophagy processes were determined by quantitative polymerase chain reaction. Autophagy flux was studied by electron microscopy, flow cytometry, immunoblotting, and immunocytochemistry. Hepatic and serum miR-29a levels were quantified by using both liver tissue and serum samples from a cohort of chronic hepatitis C virus patients and a murine CCl4 induced liver fibrosis model.
Results: In our study, we show that TGFβ and PDGF-BB resulted in decrease of intracellular miR-29a and a pronounced increase of vesicular miR-29a release into the supernatant. Strikingly, miR-29a vesicular release was accompanied by enhanced autophagic activity and up-regulation of the autophagy marker protein LC3. Moreover, autophagy inhibition strongly prevented miR-29a secretion and repressed its targets’ expression such as Col1A1. Consistently, hepatic miR-29a loss and increased LC3 expression in myofibroblastic HSC were associated with increased serum miR-29a levels in CCl4-treated murine liver fibrosis and specimens of hepatitis C virus patients with chronic liver disease.
Conclusions: We provide evidence that activation-associated autophagy in HSC induces release of miR-29a, whereas inhibition of autophagy represses fibrogenic gene expression in part through attenuated miR-29a secretion.
Background: Addition of temozolomide (TMZ) to radiotherapy (RT) improves overall survival (OS) in patients with glioblastoma (GBM), but previous studies suggest that patients with tumors harboring an unmethylated MGMT promoter derive minimal benefit. The aim of this open-label, phase III CheckMate 498 study was to evaluate the efficacy of nivolumab (NIVO) + RT compared with TMZ + RT in newly diagnosed GBM with unmethylated MGMT promoter.
Methods: Patients were randomized 1:1 to standard RT (60 Gy) + NIVO (240 mg every 2 weeks for eight cycles, then 480 mg every 4 weeks) or RT + TMZ (75 mg/m2 daily during RT and 150–200 mg/m2/day 5/28 days during maintenance). The primary endpoint was OS.
Results: A total of 560 patients were randomized, 280 to each arm. Median OS (mOS) was 13.4 months (95% CI, 12.6 to 14.3) with NIVO + RT and 14.9 months (95% CI, 13.3 to 16.1) with TMZ + RT (hazard ratio [HR], 1.31; 95% CI, 1.09 to 1.58; P = .0037). Median progression-free survival was 6.0 months (95% CI, 5.7 to 6.2) with NIVO + RT and 6.2 months (95% CI, 5.9 to 6.7) with TMZ + RT (HR, 1.38; 95% CI, 1.15 to 1.65). Response rates were 7.8% (9/116) with NIVO + RT and 7.2% (8/111) with TMZ + RT; grade 3/4 treatment-related adverse event (TRAE) rates were 21.9% and 25.1%, and any-grade serious TRAE rates were 17.3% and 7.6%, respectively.
Conclusions: The study did not meet the primary endpoint of improved OS; TMZ + RT demonstrated a longer mOS than NIVO + RT. No new safety signals were detected with NIVO in this study. The difference between the study treatment arms is consistent with the use of TMZ + RT as the standard of care for GBM.

Der Artikel „Longitudinal changes in cortical thickness in adolescents with autism spectrum disorder and their association with restricted and repetitive behaviors“ von Bieneck et al. (2021), veröffentlicht in „Genes“, beschäftigt sich mit der Frage nach einem Zusammenhang zwischen der intra-individuellen Entwicklung der Kortexdicke von Adoleszent*innen mit Autismus-Spektrum-Störung (ASS) und der Schwere restriktiver und repetitiver Verhaltensweisen (restricted and repetitive behaviors = RRB) im Verlauf dieser Lebensphase. Darüber hinaus untersuchen wir potentielle Anreicherungen von Genen, bei denen eine Assoziation mit ASS bekannt ist, in entsprechenden Hirnregionen mit abweichender Entwicklung der Kortikalen Dicke (Cortical Thickness = CT). Ziel der Studie ist es mikroskopische und makroskopische Ätiologien der ASS miteinander zu verknüpfen und diese entsprechenden klinischen Phänotypen zuzuordnen.
Die Basis dieser Forschungsarbeit bilden Daten, die im Rahmen einer longitudinalen Studie zur Gehirnentwicklung bei ASS während der Adoleszenz an der Kinder- und Jugendpsychiatrie des Universitätsklinikums in Frankfurt erhoben worden sind. Die Stichprobe setzt sich zusammen aus insgesamt 70 Proband*innen im Alter zwischen 11 und 18 Jahren zum ersten Testzeitpunkt (T1), davon N=33 mit diagnostizierter ASS und N=37 neurotypische Kontrollen. Alle Proband*innen erhalten im ersten Jahr (T1) einen strukturellen Magnetresonanztomographie-Scan (sMRT), der im Abstand von zwei Jahren (T2) wiederholt wird. Die Quantifizierung der Symptomschwere erfolgt in beiden Gruppen mittels eines klinischen Fragebogens zu restriktivem und repetitivem Verhalten an beiden Messzeitpunkten (T1, T2).
Die sMRT-Scans durchlaufen für die Auswertung ein Softwarepaket, um eine virtuelle Rekonstruktion der kortikalen Oberfläche von jedem T1-gewichteten Bild abzuleiten. Die Software gibt die intraindividuelle Veränderung der CT an jedem Vertex an. Mittels statistischer Analysen unter Verwendung eines generalisierten linearen Modells (GLM) werden Vertex-weise Unterschiede in der Veränderung der CT zwischen beiden Gruppen herausgearbeitet und mit der Änderung der Symptomschwere von RRBs korreliert.
Der dritte Schritt der Analyse umfasst die Auswertung potentieller genetischer Korrelate atypischer CT-Entwicklung der Adoleszent*innen mit ASS. Hierfür wird ein Hirnatlas (Allen Human Brain Atlas) hinzugezogen, der Informationen zur räumlichen Verteilung der Expression von Genen im menschlichen Cortex enthält. Dieser wird mit den Hirnregionen, die eine abweichende Veränderung der CT in Autist*innen zeigen, abgeglichen, und eine entsprechende Liste an Genen wird abgeleitet. Es folgt eine Analyse dieser Genliste im Hinblick auf Anreicherungen von Genen, die in Verbindung mit ASS stehen. Dieser Vorgang wird mit den Hirnregionen wiederholt, die eine Korrelation der abweichenden CT-Entwicklung mit der Veränderung der Symptomschwere von RRBs in den autistischen Teilnehmenden zeigen. Die daraus gewonnene Genliste wird auf eine vermehrte Anreicherung (i.e. Enrichment) von Genen untersucht, die in Verbindung mit restriktivem und repetitivem Verhalten stehen.
Die Ergebnisse dieser Studie zeigen eine signifikant verminderte Abnahme der CT in verschiedenen Hirnregionen von Personen mit ASS, die funktionell mit autistischen Symptomen und Verhaltensweisen gekoppelt sind. Eben diese Regionen zeigen zudem eine vermehrte Anreicherung von Genen, die ebenfalls eine Assoziation mit ASS aufweisen. Eine Verbindung von strukturellen und klinischen Parametern zeigt sich durch die Korrelation abnehmender CT in bestimmten Hirnregionen mit einer verminderten Schwere restriktiver und repetitiver Verhaltensweisen im Verlauf.
Diese Untersuchungen weisen auf Verbindungen des neuroanatomischen Parameters CT mit genetischen Grundlagen der ASS hin und zeigen einen Zusammenhang dieser Korrelationen mit unterschiedlichen Ausprägungen des klinischen autistischen Phänotyps.
Knochendefekte sind ein häufig anzutreffendes Krankheitsbild mit multiplen Ursachen wie zum Beispiel Trauma, Infektion und Tumore. Besondere Aufmerksamkeit bedürfen dabei Knochendefekte kritischer Größe (CSD). Diese sind definiert als ein Knochendefekt, der trotz adäquater Stabilisierung in der Lebzeit des Patienten nicht abheilen wird. Gerade diese sind in der Versorgung sehr zeit- und geldintensiv und stellen eine große Belastung für den Patienten dar. Trotz deren Häufigkeit finden sich nur Behandlungsverfahren, die mit signifikanten Nachteilen einhergehen. Daher ist gerade in der Behandlung von CSD weitere Forschung und die Entwicklung neuer Behandlungsmöglichkeiten nötig. Ein gut belegtes und klinisch erfolgreich angewandtes Verfahren stellt die Masquelet-Technik dar, welche ein zweizeitiges Verfahren ist, bei dem zunächst in einer ersten OP der Defekt ausgiebig durch Debridieren und die Zugabe von Antibiotika vorbereitet wird und anschließend mit einem Spacer aus Knochenzement und einem geeigneten Osteosyntheseverfahren stabilisiert wird. Über mehrere Wochen bildet sich nun eine Biomembran, die Masquelet-Membran, um den Spacer, welche in einer zweiten OP eröffnet wird. Daraufhin wird der Spacer entfernt und zum Beispiel mit autologem Knochen aus dem Beckenkamm gefüllt. Dann wird die Membran wieder verschlossen und der Defekt kann abheilen. Mit diesem Verfahren können auch große Defekte versorgt werden und in der Literatur finden sich Erfolge bei bis zu 25cm langen Defekten. Limitierungen finden sich hier in der Verfügbarkeit von autologem Knochenmaterial. Hier kann mit synthetischen Knochenersatzstoffen, wie zum Beispiel β-TCP, oder xenogenen Materialen zum Beispiel vom Rind gearbeitet werden. Einen weiteren großen Nachteil sahen wir jedoch in der Notwendigkeit einer zweiten OP. Dies setzt den Patienten erneut dem Operations- und Narkoserisiko aus, ist mit erneuten Schmerzen verbunden und führt zu verlängerten Krankenhausaufenthalten mit gesteigerten Kosten. Um dies zu umgehen, wurde in dieser Arbeit die Masquelet-Membran durch eine Membran aus dezellularisierter humaner Dermis (Epiflex) ersetzt, welche bereits erfolgreich bei der Behandlung größerer Bauchwanddefekte eingesetzt wird. So könnte man in einer einzigen OP den Defekt mit der Dermis umhüllen und direkt mit dem gewünschten Knochenersatzmaterial füllen. Hypothese war, dass eine Defektumhüllung mit Epiflex die Defektheilung vergleichbar zur induzierten Membran unterstützt. Eine weitere Fragestellung dieser Arbeit war, ob durch den Einsatz von mononukleären Zellen des Knochenmarks (BMC) die Knochenheilung zusätzlich verbessert werden kann. Zu diesem Zweck wurden sechs Gruppen aus männlichen Sprague-Dawley-Ratten gebildet, welche einen 1 cm starken Defekt im Femur gesetzt bekamen. Die Gruppen 1 und 2 wurden mit dem Masquelet-Verfahren, Gruppe 1 ohne und Gruppe 2 mit Zugabe von BMC, behandelt. Die Gruppen 3 bis 6 wurden mit dem einzeitigen Verfahren unter Verwendung der Dermis als Defektumhüllung und verschiedenen Kombinationen von BMC in einer einzigen OP behandelt. Nach 8 Wochen wurden die operierten Femora, inklusive jeweils eines nicht operierten Referenzfemur pro Gruppe explantiert und histologisch aufgearbeitet. Mittels der MOVAT Färbung wurde die Knochenneubildung im Defekt und die histologische Transformation der Membran in knöchernes Gewebe beurteilt. Es konnte ein signifikanter knöcherner Umbau der Epiflex-Membran beobachtet werden. Da mit der Epiflex-Membran ein Fremdkörper in den Organismus eingebracht wurde, wurde auch das Inflammationsgeschehen anhand histologischer Färbungen für Monozyten/Makrophagen (CD68) sowie für Granulozyten (CAE) beurteilt. Dabei zeigten sich keine signifikanten Unterschiede zwischen den Verfahren. Allerdings konnte eine unterschiedliche Verteilung der Immunzellen nachgewiesen werden. Abschließend wurde anhand von immunhistologischen α-SMA und vWF-Färbungen die Vaskularisierung im Defekt beurteilt. Die Epiflex-Membran wurde in gleichem Maße wie die induzierte Membran von Gefäßen durchdrungen. Dabei wuchsen häufig Gefäße aus dem umliegenden Muskelgewebe in den Defekt ein. Die zusätzliche Transplantation von BMC, welche eine zusätzliche Variable in dieser Arbeit darstellten, beeinflusste die Knochenheilung positiv. In Gruppen mit zusätzlichen BMC wurde geringere Inflammation und eine verbesserte Verknöcherung im Defektbereich beobachtet. Die Gefäßdichte wurde durch zusätzliche BMC jedoch nicht erhöht. Abschließend lässt sich aus den Ergebnissen schließen, dass der Ersatz der Masquelet-Membran durch die einzeitige Versorgung mit einer humanen dezellularisierten Dermis zu mindestens gleichwertigen Ergebnissen im Vergleich zur zweizeitigen induzierten Membrantechnik nach Masquelet führt.
Hintergrund und Fragestellung: Für Menschen mit Depression gestaltet sich der Zugang zu adäquater Versorgung oft schwierig. Sie müssen sich in einer äußerst komplexen Versorgungslandschaft mit vielen Behandlungsoptionen und verschiedenen Anlaufstellen orientieren, sind aber aufgrund der Depressionssymptome teilweise nicht in der Lage, sich selbst aktiv um ihre Behandlung zu kümmern. Für Frauen der ersten Einwanderergeneration konnte gezeigt werden, dass diese zwar mindestens ebenso häufig an Depression erkranken wie Menschen ohne Migrationshintergrund, aber nicht im gleichen Maße an Versorgungsangeboten teilhaben wie die restliche Bevölkerung. Ziel ist es daher, mehr darüber zu erfahren, wie Migrantinnen den Zugang zu Versorgung bei Depression erleben und welche Schwierigkeiten und Verbesserungswünsche sie haben. Es sollen konkrete Ansatzpunkte zur Verbesserung der Versorgung aufgezeigt werden.
Methodisches Vorgehen: Es wurden zwölf teilstrukturierte qualitative Einzelinterviews mit Migrantinnen der ersten Einwanderergeneration geführt, die aufgrund von Depression in Deutschland behandelt wurden. Die Auswertung der Interviews erfolgte mittels qualitativer Inhaltsanalyse.
Ergebnisse: Die Teilnehmerinnen beschreiben zahlreiche Unsicherheiten und Ängste beim Versorgungszugang, den sie als zu schwierig, kompliziert und langwierig erleben. Konkrete Schwierigkeiten sind fehlendes Wissen zu Depressionssymptomen und Behandlungsoptionen, der Umgang mit Stigmatisierung und Vorurteilen, mangelnde Behandlungskoordination und Beratung insbesondere beim Zugang zu ambulanter Psychotherapie, lange Wartezeiten sowie Verständigungsschwierigkeiten. Die Teilnehmerinnen wünschen sich bessere Beratung und Informationsmöglichkeiten zu geeigneten Behandlungsoptionen, niedrigschwelligere Behandlungsangebote, sprachliche Unterstützung insbesondere zu Beginn der Behandlung sowie Aufklärungskampagnen zum Thema Depression für die breite Öffentlichkeit.
Diskussion: Beim Versorgungszugang von Migrantinnen mit Depression besteht Verbesserungsbedarf. Die beschriebenen Schwierigkeiten und Wünsche sollten bei der Planung von Versorgungsangeboten und bei der Behandlung stärker berücksichtigt werden. Konkrete Ansatzpunkte für eine bessere Versorgung sind entstigmatisierende, öffentlichkeitswirksame Informationskampagnen zu den Symptomen und der Behandlung von Depression sowie muttersprachliche Informationsmaterialien für Patientinnen und Patienten zu Depression, den Versorgungsangeboten und den relevanten Grundlagen des Gesundheits- und Sozialsystems. Auch sprachliche Unterstützung im Einzelfall, etwa durch professionelle Sprachmittlerdienste, sollte bei Bedarf ermöglicht werden. Darüberhinaus sind individuelle Unterstützungsangebote bei der Behandlungskoordination und beim Zugang zu ambulanter Psychotherapie nötig sowie mehr Aufmerksamkeit und Einfühlungsvermögen der Behandlerinnen und Behandler für individuell bestehende Vorbehalte und Ängste von Personen mit Migrationshintergrund.
Hintergrund: Die Endometriose und die Infertilität sind beide Erkrankungsbilder, die häufig miteinander assoziiert sind. So werden Literaturen zur Folge bei mindestens 30% der Endometriose Patientinnen eine Sterilität diagnostiziert, während wiederum bei etwa 25 - 50% der Patientinnen eine Endometriose als Hauptursache des unerfüllten Kinderwunsches erkannt wird. Die Laparoskopie dient bei Patientinnen mit Endometriose als wichtiger invasiver Eingriff, der entweder zu diagnostischen Zwecken im Sinne einer Ursachenabklärung dient und / oder zu therapeutischen Zwecken bei bereits bestätigter Endometriose assoziierter Infertilität durchgeführt wird.
Problemstellung und Zielsetzung: Anhand des laparoskopischen Befundes, z.B. durch das Erfassen des rASRM Scores bei Endometriose, kann Patientinnen im Voraus keine zuverlässige Aussage über die Chance auf eine postoperative Schwangerschaft geäußert werden. Das Ziel der hier vorliegenden Arbeit ist es, den Erfolg der laparoskopischen Behandlung bei Endometriose assoziierter Infertilität unter Einbezug von Patientencharakteristika, des intraoperativen Befundes, operativer Verfahren sowie der postoperativen Fertilitätsbehandlung zu beurteilen. Auf diese Weise soll den Frauen geholfen werden, im Voraus bzw. nach laparoskopischer Behandlung der Endometriose die Chance auf eine postoperative Schwangerschaft besser abzuschätzen. Es sollen somit Faktoren ermittelt werden, die die postoperative Fertilität protektiv oder ungünstig beeinflussen.
Material und Methoden: In die Studie aufgenommen wurden alle Patientinnen (n=102), die im Zeitraum von 2007 bis 2017 an der Universitätsklinik Frankfurt am Main einen laparoskopischen Eingriff mit der Diagnose einer Endometriose erhalten haben und einen unerfüllten Kinderwunsch von mehr gleich einem Jahr äußerten. Die Datenerhebung erfolgte über die elektronische Patientenakte sowie die telefonische Befragung der Studienteilnehmerinnen zur Patientencharakteristika und der postoperativen Fertilitätsbehandlung. Die statistische Auswertung wurde durch die Nutzung des Log Rank Tests / der Kaplan Meier Methode sowie uni- und multivariater Analysen mittels
Cox Regression durchgeführt.
Ergebnisse: Von den 102 Patientinnen konnten nach einer mittleren postoperativen Nachbeobachtungszeit von 70 Monaten 71 Patientinnen (69,6%) eine Schwangerschaft verwirklichen. Dabei wurden 21,6% der Frauen durch die alleinige Laparoskopie schwanger, während bei 48% der Frauen die Schwangerschaft durch die zusätzliche Nutzung reproduktionsmedizinischer Verfahren erreicht wurde. Die meisten Schwangerschaften ereigneten sich in den ersten 6 bis 12 Monaten nach der Operation. Die mittlere Dauer bis zur Schwangerschaft betrug 10 Monate. Der Log Rank Test zeigte keine statistisch signifikanten Unterschiede in den verschiedenen rASRM Stadien bei der Betrachtung der kumulativen Schwangerschaftsrate (rASRM 1: 83,3%; rASRM 2: 66,7%; rASRM 3: 93,8%; rASRM 4: 37,9%). Die uni- und multivariaten Analysen zeigten, dass die Dauer der Infertilität (Hazard Ratio: 0,85), das Vorkommen von intestinaler Endometriose (Hazard Ratio: 0,26) und ausgeprägten Adhäsionen (Hazard Ratio: 0,45), die Anzahl der Endometriose Lokalisationen (Hazard Ratio:0,82), die Anzahl der laparoskopischen Eingriffe für Endometriose (Hazard Ratio: 0,66) sowie der Voroperationen (Hazard Ratio: 0,75) und das Vorkommen von Endometriose Rezidiven (Hazard Ratio: 0,4) die Chance auf eine Konzeption signifikant ungünstig beeinträchtigen (p< 0,05). Ein protektiver Einfluss auf die Schwangerschaft zeigte sich nur für die Koagulation am Ovar (p= 0,02; Hazard Ratio: 7,74).
Schlussfolgerung: Die laparoskopische Behandlung scheint die Erfüllung des Kinderwunsches positiv zu beeinflussen, zumal fast ¾ der Patientinnen postoperativ schwanger wurden. Höhere Schwangerschaftsraten wurden durch die zusätzliche Nutzung der assistierten Reproduktionstherapie (ART) erreicht. Aus diesem Grund sollten Frauen für eine zusätzliche postoperative ART motiviert werden. Die Patientinnen sollten darauf aufmerksam gemacht werden, dass sie sich bei einem unerfüllten Kinderwunsch früher bei ihrem Arzt vorstellen sollten. Denn je länger die Zeit bis zur Operation einer Endometriose assoziierten Infertilität beträgt, desto schlechter ist die Chance auf eine postoperative Schwangerschaft (p= 0,02). Endometriosezysten am Ovar sowie deren Entfernung schränken die klinische Fertilität nicht signifikant ein. Die Anzahl der laparoskopischen Eingriffe sowie der abdominellen Voroperationen scheinen ein wichtigerer, ungünstiger Faktor in Hinblick auf die Schwangerschaft zu sein (p = 0,04). Außerdem könnte die Koagulation am Ovar eine wichtige Behandlungsmethode bei Endometriose assoziierter Infertilität sein. Diese Operationsmethode sollte durch weitere Studien und eine größere Stichprobe untersucht werden...
Electrical stimulation (EStim) has been shown to promote bone healing and regeneration both in animal experiments and clinical treatments. Therefore, incorporating EStim into promising new bone tissue engineering (BTE) therapies is a logical next step. The goal of current BTE research is to develop combinations of cells, scaffolds, and chemical and physical stimuli that optimize treatment outcomes. Recent studies demonstrating EStim’s positive osteogenic effects at the cellular and molecular level provide intriguing clues to the underlying mechanisms by which it promotes bone healing. In this review, we discuss results of recent in vitro and in vivo research focused on using EStim to promote bone healing and regeneration and consider possible strategies for its application to improve outcomes in BTE treatments. Technical aspects of exposing cells and tissues to EStim in in vitro and in vivo model systems are also discussed.
Gait analysis as a clinical examination method has been increasingly used in recent years. In particular, the external knee adduction moment was often used as a surrogate measure for internal medial knee joint loading, e.g., in elderly individuals with medial knee osteoarthritis. Therefore, the knee adduction moment is also associated with the progression of knee osteoarthritis. Children and adolescents with valgus malalignment have been found to experience a reduced external knee adduction moment, but internal knee joint contact forces, particularly in the lateral compartment, were not previously studied.
First, medial and lateral knee joint contact forces were studied using muskulosceletal modeling in young individuals with and without valgus malalignment treated by guided growth. In addition, a systematic literature review was conducted to explore the relationship between external joint moments and internal joint contact forces. Finally, this relationship was investigated in children and adolescents with and without valgus malalignment. Furthermore, we examined whether statistical models could be determined to accurately predict internal knee joint contact forces by commonly used parameters from three-dimensional gait analysis, such as external knee joint moments.
It was found that guided growth normalized knee joint contact forces after treatment. In addition, the static radiographic mechanical axis angle correlated better after the treatment when the patients showed a typical limb alignment compared to the correlation before guided growth with the valgus malalignment due to compensating strategies during gait. Furthermore, the systematic review showed that the peak medial knee joint contact force was best predicted by the knee adduction moment and even better together with the knee flexion moment in the first half of stance. However, for the second half of stance of the medial knee joint contact force and the entire stance of the lateral knee joint contact force, only low correlations with knee adduction and/or flexion moment were found. Finally, statistical models could be determined with high accuracy for both medial and lateral knee joint contact force, for both peaks in the first and second half of stance, and for both study groups of children and adolescents with and without valgus malalignment by including knee adduction and flexion moment as predictors.
These results demonstrate the importance of examining not only the external knee adduction moment but also the knee flexion moment and, even better, the medial and lateral knee joint contact forces when evaluating knee joint loading. With these statistical models, clinicians can predict the medial and lateral knee joint contact forces without the need to perform musculoskeletal simulations and can therefore use standard three-dimensional gait analysis parameters such as knee adduction and flexion moment. This can improve guided growth treatment in children and adolescents with valgus malalignment with regard to implantation or explantation of the growth restricting plates or to rebound. Instrumented gait analysis could be particularly helpful in borderline cases, as kinematic compensation mechanisms during gait may play a role and the static radiograph alone does not provide information about dynamic joint loads.
Decoding brain states on the intrinsic manifold of human brain dynamics across wakefulness and sleep

Current state-of-the-art functional magnetic resonance imaging (fMRI) offers remarkable imaging quality and resolution, yet, the intrinsic dimensionality of brain dynamics in different states (wakefulness, light and deep sleep) remains unknown. Here we present a method to reveal the low dimensional intrinsic manifold underlying human brain dynamics, which is invariant of the high dimensional spatio-temporal representation of the neuroimaging technology. By applying this intrinsic manifold framework to fMRI data acquired in wakefulness and sleep, we reveal the nonlinear differences between wakefulness and three different sleep stages, and successfully decode these different brain states with a mean accuracy across participants of 96%. Remarkably, a further group analysis shows that the intrinsic manifolds of all participants share a common topology. Overall, our results reveal the intrinsic manifold underlying the spatiotemporal dynamics of brain activity and demonstrate how this manifold enables the decoding of different brain states such as wakefulness and various sleep stages.
Hintergrund: In den vergangenen 10 Jahren wurden an verschiedenen Epilepsiezentren in Deutschland (Bochum, Erlangen, Greifswald, Berlin Brandenburg, Frankfurt Rhein-Main) Projekte entwickelt, die sich mit telemedizinischen Arzt-zu-Arzt-Anwendungen im Bereich der Epilepsieversorgung beschäftigen.
Ziel der Arbeit: Im Folgenden wird ein Überblick über die aktuell laufenden telemedizinischen Projekte in der Epilepsieversorgung in Deutschland gegeben.
Material und Methoden: Die Verantwortlichen der einzelnen Projekte stellen ihr Projekt anhand einer vorgegebenen Struktur dar.
Ergebnisse und Diskussion: In allen Projekten konnte gezeigt werden, dass eine technische Lösung für die telemedizinische Arzt-zu-Arzt Anwendung im Bereich Epileptologie geschaffen werden kann. Die dargestellten Projekte unterscheiden sich zum Teil hinsichtlich des Zieles und der Umsetzung, zum Teil zeigen sich Übereinstimmungen. Perspektivisches Ziel ist es, aus den Erfahrungen der einzelnen Projekte eine gemeinsame Strategie zur Förderung epileptologischer Telemedizin und ihrer Überführung in die Regelversorgung zu entwickeln.
Telemedizinische Arzt-zu-Arzt-Anwendungen in der Epilepsieversorgung können helfen, die spezielle Expertise von neurologischen oder pädiatrischen EpileptologInnen flächendeckend vorzuhalten, da sie es ermöglichen, medizinische Leistung über Distanzen hinweg zu erbringen. Sowohl national als auch international werden hierzu verschiedene Lösungsansätze entwickelt. Herausforderungen begegnet man auf organisatorischer, technischer, rechtlicher und ökonomischer Ebene, sodass die langfristige Perspektive der einzelnen aktuellen Lösungsansätze noch unklar ist. Letztendlich bedarf es der Entwicklung von Betriebsmodellen, bei denen alle Akteure (Konsilgeber, Konsilanforderer, Patient, Kostenträger, Betreiber der telemedizinischen Plattform und ggf. auch die jeweilige Fachgesellschaft) jeweils den spezifischen Nutzen und die Risiken abwägen.
In dieser Arbeit wurden zur Funktionsanalyse gelagerter ThZ zwei ex vivo Messverfahren verwendet, die unterschiedliche Qualitäten der ThZ-Funktion quantifizieren. Die in vitro Aggregationsfunktion der ThZ wurde mittels Multiplate® Analyzer untersucht, die Mitochondrienfunktion mittels Oxyraph-2k.
Der Multiplate® Analyzer ist eine wohl etablierte und gut zugängliche POC-Methode, die zeitnah verlässliche Daten zur ThZ-Aggregationsfähigkeit in Vollblutproben liefert. Von Interesse war zunächst die Frage nach der Durchführbarkeit valider Multiplate®-Messungen an POOL-TK. Das für Vollblutproben konzipierte Multiplate® kann unseren Ergebnissen zufolge zur in vitro Testung von POOL-TK herangezogen werden. Die Kombination der verwendeten Mischungsverhältnisse, Suspensionsmedien und Reagenzien eignen sich zur ThZ-Stimulation außerhalb der physiologischen Umgebung des Vollbluts. In zukünftigen Studien mit höheren Fallzahlen sollte jedoch die Messgenauigkeit des Multiplate® an TK durch Anpassungen der Suspensionsmedien, sowie der ThZ- und Reagenzienkonzentration überprüft und optimiert werden. Die Entwicklung der ThZ-Funktion im Verlauf der viertägigen TK-Haltbarkeit wurde hinsichtlich der thrombozytären Aggregations- und Mitochondrienfunktion beurteilt. Weiterhin wurde der Einfluss einer kontinuierlichen Agitation durch Vergleich mit der TK-Lagerung ohne Agitation untersucht. Während die Aggregationsfähigkeit der ThZ über den viertägigen Beobachtungszeitraum überwiegend erhalten blieb, verzeichnet die mitochondriale Funktion einen signifikanten Rückgang. Unter kontinuierlicher Agitation der TK verzeichnete sich keine signifikante Abnahme der thrombozytären Aggregationsfähigkeit im Laufe der TK-Haltbarkeit. Wurden die TK ohne Agitation gelagert zeigte das Aggregationsausmaß in den ersten Tagen gegenüber den richtlinienkonform gelagerten TK keine signifikante Verschlechterung. Am vierten Tag resultierte lediglich die ThZ-Stimulation mit TRAP in einer signifikant verminderten Plättchenaggregation in der Gruppe der nicht agitierten TK. Eine Toleranz der TK bezüglich temporären Agitationspausen von ein bis zwei Tagen ist demnach anzunehmen. Nach längeren Agitationspausen ist mit signifikanten Beeinträchtigungen der Aggregationsfunktion zu rechnen.
Der Oxygraph-2k ist ein anerkanntes Messgerät zur Analyse der mitochondrialen Leistungsfähigkeit mittels hochauflösender Respirometrie. Die Daten dieser Arbeit demonstrieren eine signifikante Abnahme der mitochondrialen Leistungsfähigkeit mit - 116 -zunehmendem Alter der TK. Es zeigten sich keine Unterschiede zwischen kontinuierlich agitierten und ruhenden TK am Ende der Lagerungsperiode. Auch an den restlichen Lagerungstagen waren ruhende TK nicht verstärkt in ihrer mitochondrialen Leistung eingeschränkt, als stetig agitierte TK. Dies impliziert, dass sich die mitochondriale Leistungsfähigkeit im Laufe der TK-Alterung reduziert, ungeachtet dessen, ob die Lagerung unter der empfohlenen kontinuierlichen oder unterlassenen Agitation erfolgt. Aus den Multiplate®- und Oxygraph-Ergebnissen lässt sich schlussfolgern, dass ein TK den für die Transfusion erforderlichen Qualitätsanspruch nicht durch kurzzeitige Agitationspausen verliert. Im Klinikalltag kann es, irrtümlich sowie durch Organisationsversagen bedingt, zu unterbrochener oder gar unterlassener Agitation vor TK-Transfusion kommen. Gemäß den Ergebnissen dieser Arbeit zufolge, wird darunter jedoch die TKQualität nicht nennenswert negativ beeinflusst.
Ein weiterer Teil dieser Arbeit befasste sich mit der Untersuchung des Einflusses von Extrembedingungen, wie Kälte von 4°C oder Be- und Entschleunigungen sowie Turbulenzen beim Transport durch Rohrpostsysteme. Die Mehrzahl der Testergebnisse verzeichnete keine signifikante Beeinträchtigung der ThZ-Aggregationsfunktion. Die vorübergehende, fehlerhafte Deposition eines TK im Kühlschrank und der Rohrpostversand können somit als unbedenklich für die mittels Multiplate® und Oxygraph gemessenen TK-Qualitätsparameter angesehen werden.
Die Gesamtheit der Ergebnisse zeigt, dass kurzfristige Abweichungen von der richtlinienkonformen TK-Lagerung keine negativen Auswirkungen auf die thrombozytäre Aggregationsfähigkeit und Mitochondrienfunktion ausüben. Die Berücksichtigung dieser Erkenntnisse könnte verhindern, dass kurzzeitig fehlgelagerte TK verworfen werden, was sich positiv auf die Nutzung dieser begrenzten Ressource auswirken würde. Als Einschränkung ist zu erwähnen, dass die dargebotenen Veränderungen lediglich ex vivo ermittelt wurden. Aufgrund der Nutzung etablierter Verfahren kann angenommen werden, dass sich unsere Ergebnisse gleichsinnig auf die tatsächliche in vivo ThZ-Funktion auswirken. Der Effekt auf die in vivo ThZ-Funktion muss in zukünftigen Studien verifiziert werden. Insbesondere bleibt die Frage offen, inwiefern ein Zusammenhang zwischen einer eingeschränkten mitochondrialen Respirationsleistung und der Aggregationsfunktion besteht.
Das Capillary-Leak-Syndrom (CLS) präsentiert sich als plötzlicher Verlust von intravasaler Flüssigkeit ins Interstitium und kann als Komplikation nach einer Stammzelltransplantation (SZT) auftreten. Das Ziel der vorliegenden Studie war es, die CLS-Inzidenz, Risikofaktoren für das Auftreten und den Einfluss auf das Überleben nach SZT in einer pädiatrischen Kohorte allogener SZT-Empfänger zu bestimmen. Wir untersuchten die klinischen Berichte aller Patienten unter 18 Jahren, die in der Abteilung für Stammzelltransplantation der Klinik für Kinder- und Jugendmedizin des Universitätsklinikums Frankfurt am Main zwischen Januar 2002 und Mai 2012 allogen stammzelltransplantiert wurden. 234 Patienten erhielten 275 SZT im oben genannten Zeitraum. In 15 Fällen (5,5 %) entwickelten Patienten ein CLS.
Bedingung für die Vergabe der CLS-Diagnose war das Vorhandensein dieser drei Kriterien: Gewichtszunahme von über 3 % des Körpergewichts in 24 Stunden, positive Ein-/Ausfuhrbilanz trotz Diuretikagabe und Ödeme.
Die Wahrscheinlichkeit, ein CLS zu entwickeln war signifikant höher bei Patienten, die auch unter einer Sepsis litten (p < 0,001). Patienten mit CLS hatten zudem ein höheres Risiko innerhalb der ersten 30 Tage nach SZT an einer GvHD zu erkranken (p = 0,005).
10 der 15 CLS-Patienten mussten intensivmedizinisch behandelt werden.
CLS hängt signifikant mit dem Gesamtüberleben an Tag + 100 und Tag + 365 nach SZT zusammen (p < 0,001), indem es einen starken prädiktiven Faktor für die TRM an Tag + 100 und Tag + 365 nach SZT darstellt (p < 0,001).
CLS ist eine schwere Komplikation bei pädiatrischen SZT-Empfängern. Der biologische Zusammenhang zwischen Sepsis, GvHD und CLS-Entwicklung im Hinblick auf Zytokinfreisetzung und Endothelschaden sollte in weiteren Studien untersucht werden, um neue zielgerichtete Therapien zu unterstützen.
Background: The most common technique used worldwide to quantify blood loss during an operation is the visual assessment by the attending intervention team. In every operating room you will find scaled suction canisters that collect fluids from the surgical field. This scaling is commonly used by clinicians for visual assessment of intraoperative blood loss. While many studies have been conducted to quantify and improve the inaccuracy of the visual estimation method, research has focused on the estimation of blood volume in surgical drapes. The question whether and how scaling of canisters correlates with actual blood loss and how accurately clinicians estimate blood loss in scaled canisters has not been the focus of research to date.
Methods: A simulation study with four “bleeding” scenarios was conducted using expired whole blood donations. After diluting the blood donations with full electrolyte solution, the sample blood loss volume (SBL) was transferred into suction canisters. The study participants then had to estimate the blood loss in all four scenarios. The difference to the reference blood loss (RBL) per scenario was analyzed.
Results: Fifty-three anesthetists participated in the study. The median estimated blood loss was 500 ml (IQR 300/1150) compared to the RBL median of 281.5 ml (IQR 210.0/1022.0). Overestimations up to 1233 ml were detected. Underestimations were also observed in the range of 138 ml. The visual estimate for canisters correlated moderately with RBL (Spearman’s rho: 0.818; p < 0.001). Results from univariate nonparametric confirmation statistics regarding visual estimation of canisters show that the deviation of the visual estimate of blood loss is significant (z = − 10.95, p < 0.001, n = 220). Participants’ experience level had no significant influence on VEBL (p = 0.402).
Conclusion: The discrepancies between the visual estimate of canisters and the actual blood loss are enormous despite the given scales. Therefore, we do not recommend estimating the blood loss visually in scaled suction canisters. Colorimetric blood loss estimation could be a more accurate option.
Patient care in a neurointensive care unit (neuro-ICU) is challenging. Multidrug-resistant organisms (MDROs) are increasingly common in the routine clinical practice. We evaluated the impact of infection with MDROs on outcomes in patients with subarachnoid hemorrhage (SAH). A single-center retrospective analysis of SAH cases involving patients treated in the neuro-ICU was performed. The outcome was assessed 6 months after SAH using the modified Rankin Scale [mRS, favorable (0–2) and unfavorable (3–6)]. Data were compared by matched-pair analysis. Patient characteristics were well matched in the MDRO (n = 61) and control (n = 61) groups. In this center, one nurse was assigned to a two-bed room. If a MDRO was detected, the patient was isolated, and the nurse was assigned to the patient infected with the MDRO. In the MDRO group, 29 patients (48%) had a favorable outcome, while 25 patients (41%) in the control group had a favorable outcome; the difference was not significant (p > 0.05). Independent prognostic factors for unfavorable outcomes were worse status at admission (OR = 3.1), concomitant intracerebral hematoma (ICH) (OR = 3.7), and delayed cerebral ischemia (DCI) (OR = 6.8). Infection with MRDOs did not have a negative impact on the outcome in SAH patients. Slightly better outcomes were observed in SAH patients infected with MDROs, suggesting the benefit of individual care.
