Lieber Anti als Held : Wiederholen und 'Werden' als Leitmotive in Charles Burns' 'Black Hole'

Charles Burns’ Comic 'Black Hole', in dem die Jugendlichen einer High-School als Folge einer sexuell übertragbaren Krankheit mutieren, greift den in Superheldencomics klassischen Topos der Metamorphose auf. Im Gegensatz zu den positiven Mutationen im DC- oder Marvel-Universum, zeigt 'Black Hole' nur Deformation, die sich nicht ästhetisch oder sinnvoll auflösen lässt. Kritisch werden Erwachsenwerden und Pubertät als monströse Transformation einer Gruppe erfasst und Grenzen zwischen Realität und Phantastik, Individualisierung und Konformität, sowie sexuelle Dichotomien thematisiert. Die von Burns dargestellten Metamorphosen lassen sich mit dem Begriff des 'Werdens' fassen, mit dem Deleuze und Guattari die Idee eines strukturalistischen Ordnungssystems etablieren, das die Differenzen der einzelnen Subjekte als deren Beziehungen untereinander erfasst und diese mittels Techniken der Phantastik und der Metamorphose erkennbar zu machen vermag. Neben den Metamorphosen, ist es die Ideen der Mannigfaltigkeit des 'Werdens' und dessen infektiöse Ausbreitung, die sich analog in Burns’ Bilderwelten findet. Die Werdungs-Gruppe wird dabei nicht durch traditionelle soziale Paradigmen geprägt oder gar durch den Konsens einer homogenen Gruppe, wie sie etwa der Superheldencomic beschreibt. Indem Narrativik und Graphik die Grenzen des Medium Comics ausloten, offenbart Black Hole einen phantasmagorischen Raum der Ich-Werdung. Konventionellen Erzählformen wird eine Diachronie entgegengesetzt, die es genauer zu bestimmen gilt.
One limitation of mechanical thrombectomy (MT) is clot migration during procedure. This might be caused by abruption of the trapped thrombus at the distal access catheter (DAC) tip during stent-retriever retraction due to the cylindrical shaped tip of the DAC. Aiming to solve this problem, this study evaluates the proof-of-concept of a new designed funnel-shaped tip, in an experimental in vitro setting. Two catheter models, one with a funnel-shaped tip and one with a cylindrical-shaped tip, were compared in an experimental setup. For MT a self-made vessel model and thrombi generated from pig’s blood were used. MT was performed 20 times for each device using two different stent-retrievers, 10 times respectively. For the funnel-shaped model: for both stent-retrievers (Trevo XP ProVue 3/20 mm; Trevo XP ProVue 4/20 mm) MT was successful at first pass in 9/10 (90%), respectively. For the cylindrical-shaped model: MT was successful at first pass in 5/10 (50%) with the smaller stent-retriever and in 6/10 (60%) with the larger stent-retriever. The experiments show a better recanalization rate for funnel-shaped tips, than for cylindrical-shaped tips. These results are indicating a good feasibility for this new approach, thus the development of a prototype catheter seems reasonable.
Children’s and adolescents’ lives drastically changed during COVID lockdowns worldwide. To compare accident- and injury-related admissions to pediatric intensive care units (PICU) during the first German COVID lockdown with previous years, we conducted a retrospective multicenter study among 37 PICUs (21.5% of German PICU capacities). A total of 1444 admissions after accidents or injuries during the first lockdown period and matched periods of 2017–2019 were reported and standardized morbidity ratios (SMR) were calculated. Total PICU admissions due to accidents/injuries declined from an average of 366 to 346 (SMR 0.95 (CI 0.85–1.05)). Admissions with trauma increased from 196 to 212 (1.07 (0.93–1.23). Traffic accidents and school/kindergarten accidents decreased (0.77 (0.57–1.02 and 0.26 (0.05–0.75)), whereas household and leisure accidents increased (1.33 (1.06–1.66) and 1.34 (1.06–1.67)). Less neurosurgeries and more visceral surgeries were performed (0.69 (0.38–1.16) and 2.09 (1.19–3.39)). Non-accidental non-suicidal injuries declined (0.73 (0.42–1.17)). Suicide attempts increased in adolescent boys (1.38 (0.51–3.02)), but decreased in adolescent girls (0.56 (0.32–0.79)). In summary, changed trauma mechanisms entailed different surgeries compared to previous years. We found no evidence for an increase in child abuse cases requiring intensive care. The increase in suicide attempts among boys demands investigation.
Background: Patients undergoing allogeneic stem cell transplantation (aSCT) are at high risk to develop an invasive fungal disease (IFD). Optimisation of antifungal prophylaxis strategies may improve patient outcomes and reduce treatment costs.
Objectives: To analyse the clinical and economical impact of using continuous micafungin as antifungal prophylaxis.
Patients/Methods: We performed a single-centre evaluation comparing patients who received either oral posaconazole with micafungin as intravenous bridging as required (POS-MIC) to patients who received only micafungin (MIC) as antifungal prophylaxis after aSCT. Epidemiological, clinical and direct treatment cost data extracted from the Cologne Cohort of Neutropenic Patients (CoCoNut) were analysed.
Results: Three hundred and thirteen patients (97 and 216 patients in the POS-MIC and MIC groups, respectively) were included into the analysis. In the POS-MIC and MIC groups, median overall length of stay was 42 days (IQR: 35–52 days) vs 40 days (IQR: 35–49 days; p = .296), resulting in median overall costs of €42,964 (IQR: €35,040–€56,348) vs €43,291 (IQR: €37,281 vs €51,848; p = .993), respectively. Probable/proven IFD in the POS-MIC and MIC groups occurred in 5 patients (5%) vs 3 patients (1%; p = .051), respectively. The Kaplan-Meier analysis showed improved outcome of patients in the MIC group at day 100 (p = .037) and day 365 (p < .001) following aSCT.
Conclusions: Our study results demonstrate improved outcomes in the MIC group compared with the POS-MIC group, which can in part be explained by a tendency towards less probable/proven IFD. Higher drug acquisition costs of micafungin did not translate into higher overall costs.
