Introduction: The aim of this study was to ascertain whether the testing format of an OSPE (Objective Structured Practical Examination) in conservative dentistry (sixth semester) predicts the scores on the practical section of the state examination (11th semester) in the same subject. Taking general student profiles into consideration (score on the school-leaving exam [Abitur], score on the preliminary exam in dental medicine [Physikum], length of university study, cohorts, and sex), we also investigated if any correlations or differences exist in regard to the total and partial scores on the OSPE and the corresponding state examination.
Methods: Within the scope of this longitudinal retrospective study, exam-specific data spanning 11 semesters for dental students (N=223) in Frankfurt am Main were collected and analyzed. Statistical analysis was carried out by calculating Spearman rank correlations, partial correlations, Pearson’s correlation coefficients, and multiple regressions (SPSS Statistics 21, IBM Corporation, New York).
Results: The results show that the OSPE (Cronbach’s α=.87) correlates with level of success on the practical section of the state exam in conservative dentistry (p=.01, r=.17). Length of university study also emerged to correlate significantly with the state exam score (p=.001, r=.23). Together, these two variables contribute significantly to predicting the state exam score (p=.001, R2=.076). This was seen extensively among female students. It was also discovered that these female students had higher school-leaving exam scores than male students (F=6.09, p=.01, η2=.027), and that a significant correlation between scores on the Physikum (preliminary exam in dental medicine) and OSPE scores existed only for male students (r=.17, p=.01).
Conclusion: This study was able to demonstrate the predictive effect of a clinical OSPE regarding scores achieved on the state exam. Taking the limitations of this study into account, we are able to recommend using the OSPE testing format in the sixth semester during the clinical phase of dental study.
Einleitung: Ziel dieser Studie war es zu evaluieren, ob das Prüfungsformat einer OSPE (Objective Structured Practical Examination) durchgeführt im Fach Zahnerhaltungskunde (6. Fachsemester) den Studienerfolg im praktischen Teil des Staatsexamens (11. Fachsemester) im selben Fach prädiziert. Ferner sollte unter Berücksichtigung allgemeiner Angaben der StudienteilnehmerInnen (Abitursnote, Physikumsnote, Studiendauer, Kohorte und Geschlecht) analysiert werden, ob bezüglich der Gesamt- sowie Teilnoten der OSPE und der adäquaten Staatsexamensprüfung Zusammenhänge oder Unterschiede bestehen.
Methoden: Im Rahmen dieser longitudinalen, retrospektiven Studie wurden für einen Zeitraum von 11 Semestern prüfungsbezogene Daten von Studierenden (N=223) des Fachbereichs Zahnmedizin in Frankfurt am Main erhoben und untersucht. Für die statistische Auswertung der Daten wurden Spearman Rangkorrelationen, Partialkorrelationen, Korrelationskoeffizienten nach Pearson, und Multiple Regressionen (SPSS Statistics 21, IBM Corporation, New York) berechnet.
Ergebnisse: Die Ergebnisse zeigen, dass OSPE (Cronbachs α=.87) mit dem Erfolg im praktischen Teil des Staatsexamens im Fach Zahnerhaltungskunde korreliert (p=.01, r=.17). Als eine weitere signifikante Korrelation mit der Examensleistung erwies sich die Dauer des Studiums (p=.001, r=.23). Gemeinsam leisten diese beiden Variablen einen signifikanten Beitrag zur Vorhersage der Examensnote (p=.001, R2=.076). Das zeigte sich im größeren Umfang bei weiblichen Studierenden. Zudem wurde festgestellt, dass diese bessere Abiturnoten als männliche Studierende aufweisen (F=6.09, p=.01, η2=.027) und dass es lediglich bei männlichen Studierenden eine signifikante Korrelation zwischen der Physikumsnote (Zahnärztliche Vorprüfung) und der OSPE-Benotung gab (r=.17, p=.01).
Schlussfolgerung: In der vorliegenden Untersuchung konnte der prädiktive Effekt einer klinischen OSPE auf die Prüfungsleistung im Staatsexamen gezeigt werden. Unter Berücksichtigung der Limitation der Studie empfiehlt sich aus unserer Sicht die Durchführung eines solchen Prüfungsformats im Rahmen des klinischen Studienabschnitts im 6. Semester im Fach Zahnmedizin.
Background: While the use of plastic materials has generated huge societal benefits, the "plastic age" comes with downsides: One issue of emerging concern is the accumulation of plastics in the aquatic environment. Here, so-called microplastics (MP), fragments smaller than 5 mm, are of special concern because they can be ingested throughout the food web more readily than larger particles. Focusing on freshwater MP, we briefly review the state of the science to identify gaps of knowledge and deduce research needs.
State of the science: Environmental scientists started investigating marine (micro)plastics in the early 2000s. Today, a wealth of studies demonstrates that MP have ubiquitously permeated the marine ecosystem, including the polar regions and the deep sea. MP ingestion has been documented for an increasing number of marine species. However, to date, only few studies investigate their biological effects. The majority of marine plastics are considered to originate from land-based sources, including surface waters. Although they may be important transport pathways of MP, data from freshwater ecosystems is scarce. So far, only few studies provide evidence for the presence of MP in rivers and lakes. Data on MP uptake by freshwater invertebrates and fish is very limited.
Knowledge gaps: While the research on marine MP is more advanced, there are immense gaps of knowledge regarding freshwater MP. Data on their abundance is fragmentary for large and absent for small surface waters. Likewise, relevant sources and the environmental fate remain to be investigated. Data on the biological effects of MP in freshwater species is completely lacking. The accumulation of other freshwater contaminants on MP is of special interest because ingestion might increase the chemical exposure. Again, data is unavailable on this important issue.
Conclusions: MP represent freshwater contaminants of emerging concern. However, to assess the environmental risk associated with MP, comprehensive data on their abundance, fate, sources, and biological effects in freshwater ecosystems are needed. Establishing such data critically depends on a collaborative effort by environmental scientists from diverse disciplines (chemistry, hydrology, ecotoxicology, etc.) and, unsurprisingly, on the allocation of sufficient public funding.
Purpose: While more advanced COVID-19 necessitates medical interventions and hospitalization, patients with mild COVID-19 do not require this. Identifying patients at risk of progressing to advanced COVID-19 might guide treatment decisions, particularly for better prioritizing patients in need for hospitalization.
Methods: We developed a machine learning-based predictor for deriving a clinical score identifying patients with asymptomatic/mild COVID-19 at risk of progressing to advanced COVID-19. Clinical data from SARS-CoV-2 positive patients from the multicenter Lean European Open Survey on SARS-CoV-2 Infected Patients (LEOSS) were used for discovery (2020-03-16 to 2020-07-14) and validation (data from 2020-07-15 to 2021-02-16).
Results: The LEOSS dataset contains 473 baseline patient parameters measured at the first patient contact. After training the predictor model on a training dataset comprising 1233 patients, 20 of the 473 parameters were selected for the predictor model. From the predictor model, we delineated a composite predictive score (SACOV-19, Score for the prediction of an Advanced stage of COVID-19) with eleven variables. In the validation cohort (n = 2264 patients), we observed good prediction performance with an area under the curve (AUC) of 0.73 ± 0.01. Besides temperature, age, body mass index and smoking habit, variables indicating pulmonary involvement (respiration rate, oxygen saturation, dyspnea), inflammation (CRP, LDH, lymphocyte counts), and acute kidney injury at diagnosis were identified. For better interpretability, the predictor was translated into a web interface.
Conclusion: We present a machine learning-based predictor model and a clinical score for identifying patients at risk of developing advanced COVID-19.
Nach dem Ende des Zweiten Weltkriegs, der wohl wichtigsten historischen Zäsur im 20. Jahrhundert, herrschte mitnichten Frieden. Vielmehr wurde erstmals die bipolare Spaltung der Welt in einen kommunistischen Osten und einen demokratischen Westen sichtbar. Der Abwurf der beiden amerikanischen Atombomben 1945 auf Nagasaki und Hiroshima hatte ein atomares Wettrüsten zwischen den beiden ideologischen Blöcken eingeläutet. Dieser Konflikt wurde aber, angesichts der möglichen nuklearen Vernichtung der gesamten Menschheit, nicht mehr in einem offenen Schlagabtausch, sondern vielmehr in einem verdeckten, „kalten“ Krieg ausgetragen. In Korea, Vietnam oder Afghanistan wurden zwar konventionelle Stellvertreterkriege geführt, die eigentliche Auseinandersetzung zwischen den Vereinigten Staaten und der Sowjetunion wurde aber mit politischen, wirtschaftlichen, technologischen und nicht zuletzt auch kulturellen Drohgebärden bestritten. Inmitten dieser angespannten weltpolitischen Situation entwickelte sich im New York der 1940er und 50er Jahre eine neuartige Kunstrichtung, welche als ein radikaler Neubeginn und letztlich als erster genuin amerikanischer Stil gewertet wurde. Die Inspiration für den später als „Abstrakter Expressionismus“ betitelten Stil speiste sich zwar aus vielfältigen kulturellen Quellen, jedoch verstanden es avantgardistische Künstler wie Jackson Pollock oder Mark Rothko aus diesen eine durchweg eigenständige Formensprache auszubilden. Trotz anfänglicher Vorbehalte galt diese moderne Kunstrichtung schon Ende der 1950er Jahre als international etabliert und New York hatte der ehemaligen Kunstmetropole Paris den Rang abgelaufen. Dieser immens schnelle Aufstieg der New York School ist auf eine Reihe von begünstigenden Faktoren zurückzuführen. Durch den Krieg zur Emigration nach New York gezwungene europäische Künstler, spielten hier genauso eine wichtige Rolle wie staatliche Förderprogramme für Künstler im Rahmen des New Deal, ein enges Netzwerk von Künstlern, Kritikern, Galerien und Museen, sowie das Aufkommen der modernen Massenmedien. Von besonderer Bedeutung ist hier aber auch das Engagement verschiedener staatlicher Institutionen, welche in der – eigentlich als unpolitisch geltenden – abstrakt expressionistischen Kunst bestimmte amerikanische Eigenschaften ausgemacht hatten. Die abstrakten Gemälde konnten so als Gegenstück zum Sozialistischen Realismus der Sowjetunion inszeniert und somit letztlich als kulturelle Propagandawaffe im Kalten Krieg funktionalisiert werden. In der nachfolgenden Arbeit soll zunächst die Entwicklung der New Yorker Avantgarde nachgezeichnet werden. Anhand ausgewählter Künstler, ihrer Werke und Arbeitsweisen wird aufgezeigt, wie den abstrakten Gemälden von der damaligen Kunstkritik amerikanische Eigenschaften zugeschrieben wurden. Des Weiteren werde ich auf die Gegner der abstrakten Kunst eingehen, bevor die erfolgreiche Etablierung des Abstrakten Expressionismus durch Galerien, Museen und Massenmedien, sowie letztlich die politische Instrumentalisierung durch den CIA behandelt wird. Dabei soll auch der Tatsache Rechnung getragen werden, dass insbesondere der letztgenannte Punkt im wissenschaftlichen Diskurs über den Abstrakten Expressionismus bis heute ein höchst umstrittenes Thema darstellt. Da die folgende Arbeit interdisziplinär angelegt ist, bedient sie sich einerseits kunstgeschichtlicher Interpretationsmethoden wie der Stil- und Formanalyse zur Beschreibung der vorgestellten Werke; andererseits werden auch kulturhistorische Methoden zum Einsatz gebracht, um sich der Fragestellung aus möglichst vielen Blickwinkeln zu nähern und die Künstler und ihre Werke so in dem damaligen sozialen und politischen Kontext richtig zu verorten. Es werden sowohl ausgewählte Bildwerke analysiert als auch Statements von Künstlern, Rezensionen und Essays von zeitgenössischen Kunstkritikern, bis hin zu aktuellen kunsthistorischen Studien berücksichtigt. Da es sich beim Abstrakten Expressionismus um ein relativ kontemporäres Phänomen handelt, sind hier die Grenzen von kunstkritischer und kunsthistorischer Auseinandersetzung oftmals fließend. Auch gebietet die Komplexität des Themas ein weitgehend synchrones Vorgehen, geordnet nach den oben genannten Fragestellungen, denn eine ausschließlich chronologische Annäherung an das Thema wäre der Übersichtlichkeit der Argumentation abträglich.
Background: Critical organ shortage results in the utilization of extended donor criteria (EDC) liver grafts. These marginal liver grafts are prone to increased ischemia reperfusion injury (IRI) which may contribute to deteriorated graft function and survival. Experimental data have shown that the calcineurin inhibitor tacrolimus exerts protective effects on hepatic IRI when applied intravenously or directly as a hepatic rinse. Therefore, the aim of the present study is to examine the effects of an ex vivo tacrolimus perfusion on IRI in transplantation of EDC liver grafts.
Methods/Design: The TOP-Study (tacrolimus organ perfusion) is a randomized multicenter trial comparing the ex vivo tacrolimus perfusion of marginal liver grafts with placebo. We hypothesize that a tacrolimus rinse reduces IRI, potentially improving organ survival following transplantation of EDC livers. The study includes livers with two or more EDC, according to Eurotransplant International Foundation’s definition of EDC livers. Prior to implantation, livers randomized to the treatment group are rinsed with tacrolimus at a concentration of 20 ng/ml in 1000 ml Custodiol solution and in the placebo group with Custodiol alone. The primary endpoint is the maximum serum alanine transamninase (ALT) level within the first 48 hours after surgery; however, the study design also includes a 1-year observation period following transplantation. The TOP-Study is an investigator-initiated trial sponsored by the University of Munich Hospital. Seven other German transplant centers are participating (Berlin, Frankfurt, Heidelberg, Mainz, Münster, Regensburg, Tübingen) and aim to include a total of 86 patients.
Discussion: Tacrolimus organ perfusion represents a promising strategy to reduce hepatic IRI following the transplantation of marginal liver grafts. This treatment may help to improve the function of EDC grafts and therefore safely expand the donor pool in light of critical organ shortage.
