Membrane-Phloretin Interaction, Infrared Raman, ESR Spectroscopy The transport inhibitor phloretin was bound to human red cell membrane and the concomitant structural changes were observed by spectroscopic methods. By the spin labeling method a decrease in fluidity of the membrane was found at 1 and 10 |iM concentrations of the reagent. This result was obtained with the 2-(3-Carboxypropyl)-4,4-dimethyl-2-tridecyl-3-oxazolidinyloxyl, and the 2-(14-Carboxytetradecyl)-2-ethyl-4,4-dimethyl-3-oxazolidinyloxyl lipid spin labels. Infrared spectroscopy of modified membranes revealed an intensity increase of the POO~ band at about 1250 cm-1. Moreover, a shift of the peak at 1050 cm -1 to 1100 cm-1 was observed in the presence of phloretin. Raman spectroscopy of the membranes did not contradict the results found with infrared and ESR spectroscopy: In the phloretin modified membrane we observed a lack of the band at 1085 cm-1, which leads to suggest that the POO" and/or C-C regions are less fluid. Changes of the extracted red cell membrane lipids were less characteristic, and the results differed from those found in red cell membrane.
Divergent selection between ecologically dissimilar habitats promotes local adaptation, which can lead to reproductive isolation (RI). Populations in the Poecilia mexicana species complex have independently adapted to toxic hydrogen sulfide and show varying degrees of RI. Here, we examined the variation in the mate choice component of prezygotic RI. Mate choice tests across drainages (with stimulus males from another drainage) suggest that specific features of the males coupled with a general female preference for yellow color patterns explain the observed variation. Analyses of male body coloration identified the intensity of yellow fin coloration as a strong candidate to explain this pattern, and common-garden rearing suggested heritable population differences. Male sexual ornamentation apparently evolved differently across sulfide-adapted populations, for example because of differences in natural counterselection via predation. The ubiquitous preference for yellow color ornaments in poeciliid females likely undermines the emergence of strong RI, as female discrimination in favor of own males becomes weaker when yellow fin coloration in the respective sulfide ecotype increases. Our study illustrates the complexity of the (partly non-parallel) pathways to divergence among replicated ecological gradients. We suggest that future work should identify the genomic loci involved in the pattern reported here, making use of the increasing genomic and transcriptomic datasets available for our study system.
Die Differenzierung zwischen Teilpopulationen hin zu unterschiedlichen Arten kann nur erfolgen, wenn zwischen diesen Teilpopulationen reproduktive Isolation besteht. Wie die unterschiedlichen Arten von reproduktiver Isolation zusammenwirken und welche Voraussetzungen bestehen müssen, um neue Arten zu bilden, muss in jedem Studiensystem untersucht werden. Ein idealer Ansatzpunkt sind Arten, die sich mehrfach an anspruchsvolle Habitate angepasst haben, deren Artbildung also von ökologischen Habitatparametern bestimmt wird. Dieser Vorgang wird als Ökologische Artbildung bezeichnet. Im Artkomplex Poecilia spec., der im Süden Mexikos mehrere schwefelangepasste Ökotypen ausgebildet hat, wurden erste Hinweise auf eine Korrelation zwischen der Selektionsstärke von natürlicher und sexueller Selektion gefunden, deren Einfluss zusammen die bestehenden reproduktiven Barrieren zwischen Klarwasser- und Schwefelökotyp formen. Wie diese Reproduktionsbarrieren beschaffen sind und wie die Umweltvariable Schwefel auf die Morphologie und das Verhalten der Poeciliiden Einfluss nimmt, wurde in der vorliegenden Arbeit anhand von fünf Fragestellungen untersucht. (1) Die Körperfärbung kann ein aussagekräftiges Signal für die Qualität des potentiellen Partners bei der Fortpflanzung sein. Wie beeinflusst die extreme Umweltvariable Schwefel die Ausbildung von Färbung? (2) Sind die gefundenen Anpassungen der Färbung erblich oder werden sie plastisch entsprechend des Nahrungsangebots ausgebildet? (3) In einem der untersuchten Flusssysteme konnte unvollständige reproduktive Isolation zwischen der Klarwasser- und Schwefelpopulation nachgewiesen werden. Sind in den Mischzonen zwischen diesen beiden Habitaten Hybriden genetisch nachweisbar und bilden diese die Färbungsanpassungen der Klarwasser-, der Schwefelpopulation oder eine intermediäre Form aus? (4) Die Gelbfärbung der Flossen bei Männchen scheint ein geeignetes Merkmal für die Anzeige der Qualität zu sein, da es möglicherweise unabhängig vom Nahrungsangebot ausgebildet wird. Besteht eine weibliche Präferenz für dieses Merkmal? (5) Auch die weibliche Partnerwahlpräferenz wird vom Habitat und dem eigenen Zustand beeinflusst. Wie verändert sich die Präferenz für Männchen mit gutem Ernährungszustand bei Weibchen, die hungrig sind?
Um diese Fragen zu beantworten, wurden in mehreren Jahren Männchen und Weibchen der Arten Poecilia mexicana und Poecilia sulphuraria aus sieben Populationen im Studiengebiet in Südmexiko gefangen und auf ihre Färbung untersucht sowie Laborpopulationen getestet. Es konnten generelle Anpassungen der Färbung an die Umweltvariable Schwefel nachgewiesen werden. Dazu gehören die Aufhellung der Körperregionen, die durch Tarnung (konkret: countershading und background matching) vor Entdeckung durch Prädatoren schützen, und die Reduktion von Gelb- und Rottönen. Diese Anpassung ist vermutlich auf das geringe Angebot an Karotinoiden in den schwefelbelasteten Extremhabitaten zurückzuführen. Außerdem konnten zahlreiche flusssystem¬spezifische Anpassungen beschrieben werden, deren Ursachen in den Unterschieden zwischen den Schwefelhabitaten untereinander begründet sind. Das Flusssystem des Río Tacotalpa stellt hier eine Besonderheit dar, da Männchen eine besonders starke Gelbfärbung der Flossen aufweisen. Wildgefangene und laborgeborene Männchen dieses Flusssystems wurden verglichen, um einen Hinweis auf den Einfluss des Nahrungsangebots auf dieses Merkmal zu untersuchen. Tatsächlich ist die Ausprägung dieses Merkmals, die Gelbfärbung der Flossen, unabhängig vom Angebot an Karotinoiden. Während die hier verwendeten genetischen Analysen nicht geeignet waren, Hybriden aus den Mischzonen zwischen Schwefel- und Klarwasserhabitat nachzuweisen, ergaben die Untersuchungen von Individuen aus den Mischzonen keine eindeutigen Ergebnisse über eine etwaige intermediäre Ausbildung der Färbung. Die Präsentation von Männchen, deren Gelbintensität an den Flossenspitzen künstlich verändert wurde, konnte bei Weibchen keine eindeutige Präferenz für stärker gefärbte Männchen aufzeigen. Vielmehr weist dieses Ergebnis auf eine starke Korrelation zwischen mehreren Merkmalen (z. B. weitere morphologische Merkmale, Verhalten) hin, die für die Beurteilung der männlichen Qualität herangezogen werden. Die weibliche Präferenz für konditionsabhängige Merkmale wird bei schwefelangepassten Weibchen leicht verstärkt, wenn diese hungrig sind. Eine solche flexible Präferenz sollte gerade in Habitaten mit starken Fluktuationen im Nährstoffangebot existieren. Dabei waren Weibchen, denen Videoaufnahmen präsentiert wurden, eher in der Lage, das qualitativ hochwertigere Männchen zu identifizieren, als Weibchen, denen animierte Bilder präsentiert wurden. Auch hier wird davon ausgegangen, dass die Reduktion auf eines oder wenige Merkmale, die für die Partnerwahl zur Verfügung stehen, keine ausreichend starke Reaktion auslösen können. Vielmehr ist der Zugriff auf alle Aspekte der männlichen Erscheinung wichtig, um die Qualität des potentiellen Partners zu beurteilen.
Färbung ist also generell geeignet, den Ökotyp eines Individuums zu bestimmen und ein solches Merkmal kann der Artbestimmung im ersten Schritt der Partnerwahl dienen. Dasjenige männliche Färbungsmerkmal, das über mehrere Generationen gleichbleibend ausgeprägt wurde – die Gelbfärbung der Flossen – reicht jedoch nicht aus, um bei der weiblichen Partnerwahl eine Reaktion auszulösen. Vielmehr deuten die Ergebnisse auf eine enge Korrelation der Färbung mit weiteren Merkmalen in Morphologie und Verhalten eines Individuums hin, die vom wählenden Weibchen stets gemeinsam entsprechend der Multiple-message-Theorie betrachtet werden. Auch der Vergleich zwischen Videoaufnahmen und animierten Fotografien als Stimuli bei der Partnerwahl ergab, dass der Aspekt Verhalten (nur verfügbar mit Videoaufnahmen) für eine Partnerwahlentscheidung von Bedeutung ist.
Meine Arbeit konnte den bestehenden Wissensschatz um die bestehenden reproduktiven Barrieren im Studiensystem um den Aspekt der Färbung erweitern. Meine Ergebnisse zeigen weitere spannende Fragestellungen auf. Je größer das Verständnis der vorliegenden Selektionskräfte und Mechanismen reproduktiver Isolation ist, desto besser kann die Wissenschaft verstehen, welche Umgebungsvariablen welchen Einfluss auf den Prozess der Artbildung haben.
This paper is the first to conduct an incentive-compatible experiment using real monetary payoffs to test the hypothesis of probabilistic insurance which states that willingness to pay for insurance decreases sharply in the presence of even small default probabilities as compared to a risk-free insurance contract. In our experiment, 181 participants state their willingness to pay for insurance contracts with different levels of default risk. We find that the willingness to pay sharply decreases with increasing default risk. Our results hence strongly support the hypothesis of probabilistic insurance. Furthermore, we study the impact of customer reaction to default risk on an insurer’s optimal solvency level using our experimentally obtained data on insurance demand. We show that an insurer should choose to be default-free rather than having even a very small default probability. This risk strategy is also optimal when assuming substantial transaction costs for risk management activities undertaken to achieve the maximum solvency level.
The article presents the results of numerical and experimental investigations of guided wave propagation in aluminum plates with variable thickness. The shapes of plate surfaces have been specially designed and manufactured using a CNC milling machine. The shapes of the plates were defined by sinusoidal functions varying in phase shift, which forced the changes in thickness variability alongside the propagation path. The main aim of the study is to analyze the wave propagation characteristics caused by non-uniform thickness. In the first step, the influence of thickness variability on the time course of propagating waves has been analyzed theoretically. The study proves that the wave propagation signals can be determined based on knowledge about the statistical description of the specimen geometry. The histograms of thickness distribution together with the a priori knowledge of the dispersion curves were used to develop an iterative procedure assuming that the signal from the previous step becomes the excitation in the next step. Such an approach allowed for taking into account the complex geometry of the plate and rejecting the assumption about the constant average thickness alongside the propagation path. In consequence, it was possible to predict correctly the signal time course, as well as the time of flight and number of propagating wave modes in specimens with variable thickness. It is demonstrated that theoretical signals predicted in this way coincide well with numerical and experimental results. Moreover, the novel procedure allowed for the correct prediction of the occurrence of higher-order modes.
The article investigates the results obtained from numerical simulations and experimental tests concerning the propagation of guided waves in corroded steel plates. Developing innovative methodologies for assessing corrosion-induced degradation is crucial for accurately diagnosing offshore and ship structures exposed to harsh environmental conditions. The main aim of the research is to analyze how surface irregularities affect wave propagation characteristics. An investigation was conducted for antisymmetric fundamental mode A0. Specifically, the study examines the asymmetrical wavefronts generated by nonuniform thickness in damaged specimens. Initially, numerical analysis explores the impact of thickness variation on wave field symmetry. Corroded plates with varying levels of degradation are modeled using the random fields approach, with degradation levels ranging from 0 % to 60 %. Subsequently, the research investigates how the standard deviation of thickness distribution (from 5 % to 20 % of the initial thickness) and excitation frequency (from 50 to 150 kHz) influence recorded signals and the shape of reconstructed wavefronts. Each scenario compares wavefront symmetry levels estimated using rotational and bilateral symmetry degrees as indicative parameters. The numerical simulations are complemented by experimental tests conducted on plates with three different degradation levels. The results demonstrate the efficacy of the proposed wave field analysis approach for assessing structural integrity, as evidenced by the agreement between numerical predictions and experimental observations.
In this paper we propose a sociological concept of innovation capable of transcending the limitations faced by the approaches of common theories of action. The concept was formulated by Ulrich Oevermann and is based upon Max Weber’s theory of charismatic authority. We apply this concept to archaeological data, using the example of Neolithic copper metallurgy in central Europe, and discuss the importance of analyzing innovations that failed to materialize even though they might have been "in the air" at the time. The concept sketched here enables the scientific study of such a phenomenon.
Die randomisierte, dreiarmig kontrollierte Studie zu täglicher, peroraler Zusatzkost (ONS) bei Hämodialysepatienten (CHD) im Endstadium der Niereninsuffizienz (ESRD) über 6 Monate zeigte keine signifikanten Verbesserungen hinsichtlich folgender Nutritions-/Retentions- und Inflammationsparameter: Subjective Global Assessment (SGA); Body Mass Index (BMI); Querschnitt des Muskulus Iliopsoas,Oberarmumfang und Dicke des Unterhautfettgewebes (MRT); örperzellmasse und Phasenwinkel (Bioimpedanzanalyse BIA); Tumornekrosefaktor α (TNFα); Interleukin 1β und 6 (IL-1β und IL-6); C-Reaktives Protein (CRP). Der Querschnitt des Muskulus biceps brachii blieb in der Kontrollgruppe anfangs und zum Ende höher wie in den Interventionsgruppen. Der Serumkreatininwert der Interventionsgruppe mit HIV war anfangs geringer als in den übrigen Gruppen, die glomeruläre Filtrationsrate entsprechend besser, zum Ende waren diese Unterschiede nivelliert. Der Hauptbefund liegt in der hohen Mortalitätsrate der HIV-positiven Hämodialysepatienten (2 von 7 Pat., 28,6%), von denen beide im SGA als schwer mangel-/fehlernährt eingestuft wurden. Die Therapie eines Malnutritions-Infalmmations-Komplexes ist nicht allein durch orale Zusatzkost möglich. Weitere Studien müssen multimodale Konzepte zur Diagnose und zur Therapie erforschen. Hierzu kann perorale Zusatzkost ein einfach durchzuführendes Mittel als Teil der Behandlungsstrategie sein, zur erweiterten Diagnose kann die Bioimpedanzanalyse eine Möglichkeit sein, um den Teilaspekt der Nutritionskontrolle zu erfüllen.
The advent of improved experimental and theoretical techniques has brought a lot of attention to the electric dipole (E1) response of atomic nuclei in the last decade. The extensive studies have led to the observation and interpretation of a concentration of E1 strength energetically below the Giant Dipole Resonance in many nuclei. This phenomenon is commonly denoted as Pygmy Dipole Resonance (PDR). This contribution will summarize the most important results obtained using different experimental probes, define the challenges to gain a deeper understanding of the excitations, and discuss the newest experimental developments.
With the introduction of the virtual allocation crossmatch in the Eurotransplant (ET) region in 2023, the determination of unacceptable antigen mismatches (UAM) in kidney transplant recipients is of utmost importance for histocompatibility laboratories and transplant centers. Therefore, a joined working group of members from the German Society for Immunogenetics (Deutsche Gesellschaft für Immungenetik, DGI) and the German Transplantation Society (Deutsche Transplantationsgesellschaft, DTG) revised and updated the previous recommendations from 2015 in light of recently published evidence. Like in the previous version, a wide range of topics is covered from technical issues to clinical risk factors. This review summarizes the evidence about the prognostic value of contemporary methods for HLA antibody detection and identification, as well as the impact of UAM on waiting time, on which these recommendations are based. As no clear criteria could be determined to differentiate potentially harmful from harmless HLA antibodies, the general recommendation is to assign all HLA against which plausible antibodies are found as UAM. There is, however, a need for individualized solutions for highly immunized patients. These revised recommendations provide a list of aspects that need to be considered when assigning UAM to enable a fair and comprehensible procedure and to harmonize risk stratification prior to kidney transplantation between transplant centers.
Plexins are widely expressed transmembrane proteins that mediate the cellular effects of semaphorins. The molecular mechanisms of plexin-mediated signal transduction are still poorly understood. Here we show that signalling via B-family plexins leading to the activation of the small GTPase RhoA requires activation of the IκB kinase (IKK)-complex. In contrast, plexin-B-dependent regulation of R-Ras activity is not affected by IKK activity. This regulation of plexin signalling depends on the kinase activity of the IKK-complex, but is independent of NF-κB activation. We confirm that the IKK-complex is active in tumour cells and osteoblasts, and we demonstrate that plexin-B-dependent tumour cell invasiveness and regulation of osteoblast differentiation require an active IKK-complex. This study identifies a novel, NF-κB-independent function of the IKK-complex and shows that IKK directs plexin-B signalling to the activation of RhoA.
Loperamide, pimozide, and STF-62247 trigger autophagy-dependent cell death in glioblastoma cells

Autophagy is a well-described degradation mechanism that promotes cell survival upon nutrient starvation and other forms of cellular stresses. In addition, there is growing evidence showing that autophagy can exert a lethal function via autophagic cell death (ACD). As ACD has been implicated in apoptosis-resistant glioblastoma (GBM), there is a high medical need for identifying novel ACD-inducing drugs. Therefore, we screened a library containing 70 autophagy-inducing compounds to induce ATG5-dependent cell death in human MZ-54 GBM cells. Here, we identified three compounds, i.e. loperamide, pimozide, and STF-62247 that significantly induce cell death in several GBM cell lines compared to CRISPR/Cas9-generated ATG5- or ATG7-deficient cells, pointing to a death-promoting role of autophagy. Further cell death analyses conducted using pharmacological inhibitors revealed that apoptosis, ferroptosis, and necroptosis only play minor roles in loperamide-, pimozide- or STF-62247-induced cell death. Intriguingly, these three compounds induce massive lipidation of the autophagy marker protein LC3B as well as the formation of LC3B puncta, which are characteristic of autophagy. Furthermore, loperamide, pimozide, and STF-62247 enhance the autophagic flux in parental MZ-54 cells, but not in ATG5 or ATG7 knockout (KO) MZ-54 cells. In addition, loperamide- and pimozide-treated cells display a massive formation of autophagosomes and autolysosomes at the ultrastructural level. Finally, stimulation of autophagy by all three compounds is accompanied by dephosphorylation of mammalian target of rapamycin complex 1 (mTORC1), a well-known negative regulator of autophagy. In summary, our results indicate that loperamide, pimozide, and STF-62247 induce ATG5- and ATG7-dependent cell death in GBM cells, which is preceded by a massive induction of autophagy. These findings emphasize the lethal function and potential clinical relevance of hyperactivated autophagy in GBM.
Tiotropium as an add-on treatment option for severe uncontrolled asthma in preschool patients

Background: Toddlers with asthma suffer disproportionally more than school-aged children from exacerbations with emergency visits and hospital admissions despite inhaled corticosteroid (ICS) treatment. A recent trial for children ≤ 5 years showed tolerability of tiotropium and potential to reduce asthma-related events.
Methods: We conducted a retrospective analysis of electronic outpatient records (2017‒2019) of children < 6 years treated with ICS plus long-acting β2-agonists (LABAs) plus tiotropium as an add-on for uncontrolled severe asthma. The primary endpoint was a comparison of systemic corticosteroid (SCS) prescriptions 6 months before and after ICS/LABA/tiotropium start. Secondary endpoints included physician visits, hospitalisations and antibiotic prescriptions. We compared outcomes with children without asthma matched for age, sex, season and screening date.
Results: Compared with a mean 2.42 (95% CI: 1.75, 3.36) SCS courses per patient within 6 months prior to ICS/LABA/tiotropium, 0.74 (95% CI: 0.25, 1.08) SCS courses per patient were prescribed within 6 months after starting ICS/LABA/tiotropium (P< 0.001). Physician visits dropped from 9.23 (95% CI: 7.15, 12.72) to 5.76 (95% CI: 3.10, 7.70) per patient (P< 0.01). Nineteen hospitalisations were recorded 6 months before ICS/LABA/tiotropium compared with one hospitalisation after (P< 0.01). A mean 1.79 antibiotic courses (95% CI: 1.22, 2.23) per patient were prescribed before ICS/LABA/tiotropium compared with 0.74 (95% CI: 0.22, 1.00) after ICS/LABA/tiotropium (P< 0.001). Hospitalisation rates for patients at observation end were not statistically different from healthy controls before/after matching.
Interpretation: Our retrospective study showed that adding tiotropium to ICS/LABA is a new treatment option for patients with severe preschool asthma; however, larger confirmatory studies are needed.
Strong dose response after immunotherapy with PQ grass using conjunctival provocation testing

Background: Pollinex Quattro Grass (PQ Grass) is an effective, well-tolerated, short pre-seasonal subcutaneous immunotherapy to treat seasonal allergic rhinoconjunctivitis (SAR) due to grass pollen. In this Phase II study, 4 cumulative doses of PQ Grass and placebo were evaluated to determine its optimal cumulative dose.
Methods: Patients with grass pollen-induced SAR were randomised to either a cumulative dose of PQ Grass (5100, 14400, 27600 and 35600 SU) or placebo, administered as 6 weekly subcutaneous injections over 31–41 days (EudraCT number 2017-000333-31). Standardized conjunctival provocation tests (CPT) using grass pollen allergen extract were performed at screening, baseline and post-treatment to determine the total symptom score (TSS) assessed approximately 4 weeks after dosing. Three models were pre-defined (Emax, logistic, and linear in log-dose model) to evaluate a dose response relationship.
Results: In total, 95.5% of the 447 randomized patients received all 6 injections. A highly statistically significant (p < 0.0001), monotonic dose response was observed for all three pre-specified models. All treatment groups showed a statistically significant decrease from baseline in TSS compared to placebo, with the largest decrease observed after 27600 SU (p < 0.0001). The full course of 6 injections was completed by 95.5% of patients. Treatment-emergent adverse events were similar across PQ Grass groups, and mostly mild and transient in nature.
Conclusions: PQ Grass demonstrated a strong curvilinear dose response in TSS following CPT without compromising its safety profile.
Patients with ataxia-telangiectasia (A-T) suffer from progressive cerebellar ataxia, immunodeficiency, respiratory failure, and cancer susceptibility. From a clinical point of view, A-T patients with IgA deficiency show more symptoms and may have a poorer prognosis. In this study, we analyzed mortality and immunity data of 659 A-T patients with regard to IgA deficiency collected from the European Society for Immunodeficiencies (ESID) registry and from 66 patients with classical A-T who attended at the Frankfurt Goethe-University between 2012 and 2018. We studied peripheral B- and T-cell subsets and T-cell repertoire of the Frankfurt cohort and survival rates of all A-T patients in the ESID registry. Patients with A-T have significant alterations in their lymphocyte phenotypes. All subsets (CD3, CD4, CD8, CD19, CD4/CD45RA, and CD8/CD45RA) were significantly diminished compared to standard values. Patients with IgA deficiency (n = 35) had significantly lower lymphocyte counts compared to A-T patients without IgA deficiency (n = 31) due to a further decrease of naïve CD4 T-cells, central memory CD4 cells, and regulatory T-cells. Although both patient groups showed affected TCR-ß repertoires compared to controls, no differences could be detected between patients with and without IgA deficiency. Overall survival of patients with IgA deficiency was significantly diminished. For the first time, our data show that patients with IgA deficiency have significantly lower lymphocyte counts and subsets, which are accompanied with reduced survival, compared to A-T patients without IgA deficiency. IgA, a simple surrogate marker, is indicating the poorest prognosis for classical A-T patients. Die Häufigkeitsrate atopischer Erkrankungen bei Kindern, wie Heuschnupfen, Asthma, Neurodermitis (atopische Dermatitis), nimmt weltweit zu. Die Gründe sind vielschichtig. Gesichert ist der Zusammenhang zwischen der erblichen Überempfindlichkeit gegenüber natürlichen Substanzen (Atopie) und vermehrter Allergen- und Passivrauch-Exposition sowie Zunahme der Ein-Kind-Familien, Veränderung der mikrobiologischen Besiedlung des Darmes und Infektexposition. Besonders gut untersucht wurden diese Zusammenhänge von Erika von Mutius in einer Studie, in der sie von 1991 bis 1992 die Häufigkeit von Asthma in München (5030 Kinder) und Leipzig/Bitterfeld (2623 Kinder) verglichen hat.
Introduction: Cesarean section (CS) rates are increasing worldwide. One constant indication is the breech presentation at term. By offering external cephalic version (ECV) and vaginal breech delivery CS rates can be further reduced. Objective: This study aimed to analyze the ECV at 38 weeks of gestation with the associate uptake rate, predicting factors, success rate, and complications at a tertiary healthcare provider in Germany specializing in vaginal breech delivery. Methods: We conducted a prospective cohort study with retrospective data acquisition. All women with a singleton fetus in breech presentation presenting after 34 weeks of gestation for counseling between 2013 and 2017 were included. ECV impact factors were analyzed using logistic regression. Results: A total of 1,598 women presented for breech birth planning. ECV was performed on 353 patients. The overall success rate was 22.4%. A later week of gestation (odds ratio [OR] 1.69), an abundant amniotic fluid index (AFI score) (OR 5.74), fundal (OR 3.78) and anterior (OR 0.39) placental location, and an oblique lie (OR 9.08) were significantly associated with successful ECV in our population. No major complications were observed. The overall vaginal delivery rates could be increased to approximately 14% with ECV. Conclusion: The demand for alternative birth modes other than CS for breech birth is high in the area of Frankfurt, Germany. Our study offers evidence of the safety of ECV at 38 weeks. Centers with expertise in vaginal breech delivery and ECV can reduce CS-rates. To further establish vaginal breech delivery and ECV as alternate options, the required knowledge and skill should be implemented in the revised curricula.
