Poster presentation: Introduction Adequate anesthesia is crucial to the success of surgical interventions and subsequent recovery. Neuroscientists, surgeons, and engineers have sought to understand the impact of anesthetics on the information processing in the brain and to properly assess the level of anesthesia in an non-invasive manner. Studies have indicated a more reliable depth of anesthesia (DOA) detection if multiple parameters are employed. Indeed, commercial DOA monitors (BIS, Narcotrend, M-Entropy and A-line ARX) use more than one feature extraction method. Poster presentation: Functional connectivity of the brain describes the network of correlated activities of different brain areas. However, correlation does not imply causality and most synchronization measures do not distinguish causal and non-causal interactions among remote brain areas, i.e. determine the effective connectivity [1]. Identification of causal interactions in brain networks is fundamental to understanding the processing of information. Attempts at unveiling signs of functional or effective connectivity from non-invasive Magneto-/Electroencephalographic (M/EEG) recordings at the sensor level are hampered by volume conduction leading to correlated sensor signals without the presence of effective connectivity. Here, we make use of the transfer entropy (TE) concept to establish effective connectivity. The formalism of TE has been proposed as a rigorous quantification of the information flow among systems in interaction and is a natural generalization of mutual information [2]. In contrast to Granger causality, TE is a non-linear measure and not influenced by volume conduction. ...
Die primär – und sekundärprophylaktische Versorgung durch einen plötzlichen Herztod bedrohter Patienten mittels Implantierbarer Cardioverter Defibrillatoren hat sich in mehreren groß angelegten Studien einer rein konservativ-medikamentösen Behandlung gegenüber als überlegen erwiesen. Ein Hauptproblem dieser Therapieform ist jedoch die mit 25 – 40% berichtete hohe Inzidenz inadäquater Therapieabgaben. Bisher weitgehend ungeklärt blieben die prognostische Bedeutung, eventuelle Risikofaktoren sowie die daraus abzuleitenden therapeutischen Implikationen des Auftretens inadäquater Therapieabgaben. Die vorliegende Arbeit analysiert die Daten 783 (630 Männer, 153 Frauen. Medianes Alter 65 Jahre) konsekutiv im Zeitraum von 1993 bis 2005 in der Medizinischen Klinik III – Kardiologie und Angiologie – Abteilung für Elektrophysiologie der Universitätsklinik der Johann Wolfgang Goethe-Universität implantierter Patienten über einen medianen Nachbeobachtungszeitraum von 37 Monaten. Implantiert wurden nach allgemein anerkannter Indikationsstellung Cardioverter Defibrillatoren der Firmen CPI/Guidant, Ela-Medical, Medtronic Inc. und St. Jude Medical. Zum Einsatz kamen hierbei Ein – Zwei – und Dreikammergeräte (Einkammer = 511, Zweikammer = 239, Dreikammer = 33). Die Sachkontrolluntersuchungen fanden im ersten Jahr nach Implantation dreimonatig, danach halbjährlich sowie bei Notwendigkeit außerplanmäßig statt. Hierbei wurden jeweils alle stattgehabten, von den Implantierbaren Cardioverter Defibrillatoren registrierten Ereignisse ausgedruckt und auf Speichermedien gesichert. Die archivierten Elektrogramme der in diesem Zeitraum aufgetretenen 11.942 tachykarden Ereignisse bilden die Grundlage der Analyse der vorliegenden Arbeit. Hierbei wurden alle aufgetreten inadäquaten Therapiegaben an Hand der Elektrogramme einer von neun ätiologischen Kategorien zugeordnet. Der aktuelle Status aller 783 Patienten wurde im Rahmen der vorliegenden Arbeit, soweit unbekannt und somit notwendig, telefonisch oder per Briefkontakt mit den weiterbehandelnden Ärzten und Zentren eruiert. Im Rahmen der Untersuchung wurden Parameter betreffend der Erkrankungen der Patienten (Herzinsuffizienz, Koronare Herzerkrankung, Dilatative Cardiomyopathie, Klappenvitien und vorbekannte atriale Tachykardien) sowie zu ihrer zum jeweiligen Untersuchungszeitpunkt aktuellen Medikation (Klasse-I-Antiarrhythmika, Sotalol, Amiodaron, Beta-Blocker, Klasse-IV-Antiarrhythmika, Digitalisglykoside, ACE-Inhibitoren/AT1-Antagonisten, Aldosteronantagonisten, Diuretika und CSE-Inhibitoren ) erfasst. Die statistische Auswertung bezüglich des Auftretens inadäquater Therapien sowie ihrer Ätiologie, der Versterbenshäufigkeit, der Überlebensdauer, des gefährdeten Auftretenszeitraums inadäquater Therapien, eines eventuellen prädilektiven Risikoalters, des Einflusses der verschieden Ätiologien und der Anzahl inadäquater Therapiegaben auf Überlebensdauer und Versterbenshäufigkeit, des Einflusses der Begleitmedikation, des Einflusses der Begleiterkrankungen, der Differenzen zwischen Ein – Zwei – und Dreikammergeräten, der Differenzen der Geräte der verschiedenen Hersteller sowie der erfolgten Programmierung der Implantierbaren Cardioverter Defibrillatoren erfolgte mittels des Leevene-Tests, des Kolmogoroff-Smirnoff-Tests, des Zweistichproben-t-Tests sowie Fisher’s exaktem Test. Im nachbeobachteten Zeitraum wurden 11.942 tachykarde Ereignisse registriert, deren Auswertung 2806 (23,5%) inadäquate Therapien ergab. Davon waren 27,9% Defibrillationen, 72,1% antitachykarde Überstimulationen. 200 der 783 Patienten (25,5%) erlitten inadäquate Therapieabgaben während der Nachbeobachtungsperiode. 29,1% der implantierten CPI/Guidant-Geräte waren von inadäquaten Therapien betroffen, 40,8% der Ela90 Geräte, 21,2% der Medtronic-Geräte und 25% der St. Jude-Geräte. Tachykard übergeleitetes Vorhofflimmern machte 45,6% der inadäquaten Therapien aus, Sinustachykardien 11,3%, Vorhofflattern 9,1%, supraventrikuläre Tachykardien 16,1%, Sondendefekte 14,1%, Muskelpotentialfehlwahrnehmungen 0,8%, T-Wellen-Fehlwahrnehmungen 1,6%, R-Wellen-Doppelwahrnehmungen 0,4% und nicht anhaltende Ventrikuläre Tachykardien 0,9%. Die Analyse der Differenzen der Patienten, welche inadäquate Therapien erlitten und der Patienten, welche keine inadäquaten Therapien erlitten, ergab signifikante Unterschiede. Die Nachbeobachtungsperiode der Patienten mit inadäquaten Therapien war mit 54,2 Monaten im Median länger als die der Patienten ohne inadäquate Therapien (Median hier 30 Monate, p<0,000001). Gleiches gilt für die Überlebensdauer der Patienten mit inadäquaten Therapien (mediane Überlebenszeit 56,9 Monate versus 33,4 Monate, p<0,0001). Weder das generelle Auftreten inadäquater Therapien noch die Zahl der erlittenen inadäquaten Therapien reduzierten die Überlebensdauer. Auch konnte keine Form von inadäquater Therapie identifiziert werden, die mit einer Übersterblichkeit einherging. Das Auftreten inadäquater Therapien an sich jedoch ging in einer Subgruppenanalyse der Patienten mit inadäquaten Therapien, der Patienten mit nur adäquaten Therapien und der Patienten ohne jegliche Therapien mit einer erhöhten Versterbenswahrscheinlichkeit einher (p<0,000003 bzw. p<0,01). Insofern kann an dieser Stelle ein negativer Einfluss inadäquater Therapien in der Zusammenschau der Befunde nur gemutmaßt, nicht aber eindeutig nachgewiesen werden. Bezüglich der Risikofaktoren für das Auftreten inadäquater Therapien ergab sich ein gefährdeter Zeitraum innerhalb der ersten sechs Monate nach Implantation, was auf die zu diesem Zeitpunkt noch nicht individuell optimierte Programmierung zurückzuführen ist. Patienten jeden Alters wiesen ein vergleichbares Risiko auf, inadäquate Therapien zu erleiden. Insofern konnte kein Risikoalter identifiziert werden. Mit einem erhöhten Risiko für die Entwicklung inadäquater Therapien gingen eine insuffiziente Versorgung mit Betablockern oder Amiodaron (p<0,000001 bzw. p<0,04) sowie vor der Implantation bekannte atriale Tachykardien (p<0,001) einher. Beide Umstände erklären sich mit der Dominanz atrialer Rhythmusstörungen als auslösende Ursache inadäquater Therapien (82,1% aller inadäquaten Therapien wurden durch atriale Rhythmusstörungen ausgelöst) sowie durch den reduzierenden Effekt der beiden Antiarrhythmika auf die Inzidenz dieser atrialen Rhythmusstörungen. Die restlichen Begleiterkrankungen blieben ohne nachweisbaren Einfluss auf das Auftreten inadäquater Therapieabgaben. Zwei- und Dreikammergeräte konnten, entgegen der Erwartungen, die Inzidenz inadäquater Therapieabgaben gegenüber den Einkammergeräten nicht signifikant reduzieren (p<0,1). Am ehesten zu erklären ist dies mit den bei Zwei- und Dreikammergeräten vermehrt auftretenden Wahrnehmungsproblemen sowie den zusätzlich benötigten, defektanfälligen Sonden. Die vorliegende Arbeit konnte die berichtete hohe Inzidenz inadäquater Therapiegaben sowie deren dominierende Ätiologien in einem breiten, den klinischen Routinebedingungen entsprechenden Patientenkollektiv innerhalb eines langen Zeitverlaufes bestätigen und eventuelle Risikofaktoren identifizieren. Weiterhin konnten Erkenntnisse bezüglich der prognostischen Bedeutung inadäquater Therapieabgaben gewonnen werden. Die Kenntnis der Risikofaktoren könnte dazu beitragen helfen, gefährdete Patienten frühzeitig zu identifizieren sowie durch prophylaktische Maßnahmen die Inzidenz inadäquater Therapiegaben zu mindern. Weiterhin können die in dieser Arbeit gewonnen Erkenntnisse als Grundlage der Hypothesengenerierung und Planung weiterer klinischer Studien dienen.
Poster presentation: Introduction The ability of neurons to emit different firing patterns is considered relevant for neuronal information processing. In dopaminergic neurons, prominent patterns include highly regular pacemakers with separate spikes and stereotyped intervals, processes with repetitive bursts and partial regularity, and irregular spike trains with nonstationary properties. In order to model and quantify these processes and the variability of their patterns with respect to pharmacological and cellular properties, we aim to describe the two dimensions of burstiness and regularity in a single model framework. Methods We present a stochastic spike train model in which the degree of burstiness and the regularity of the oscillation are described independently and with two simple parameters. In this model, a background oscillation with independent and normally distributed intervals gives rise to Poissonian spike packets with a Gaussian firing intensity. The variability of inter-burst intervals and the average number of spikes in each burst indicate regularity and burstiness, respectively. These parameters can be estimated by fitting the model to the autocorrelograms. This allows to assign every spike train a position in the two-dimensional space described by regularity and burstiness and thus, to investigate the dependence of the firing patterns on different experimental conditions. Finally, burst detection in single spike trains is possible within the model because the parameter estimates determine the appropriate bandwidth that should be used for burst identification. Results and Discussion We applied the model to a sample data set obtained from dopaminergic substantia nigra and ventral tegmental area neurons recorded extracellularly in vivo and studied differences between the firing activity of dopaminergic neurons in wildtype and K-ATP channel knock-out mice. The model is able to represent a variety of discharge patterns and to describe changes induced pharmacologically. It provides a simple and objective classification scheme for the observed spike trains into pacemaker, irregular and bursty processes. In addition to the simple classification, changes in the parameters can be studied quantitatively, also including the properties related to bursting behavior. Interestingly, the proposed algorithm for burst detection may be applicable also to spike trains with nonstationary firing rates if the remaining parameters are unaffected. Thus, the proposed model and its burst detection algorithm can be useful for the description and investigation of neuronal firing patterns and their variability with cellular and experimental conditions.
Poster presentation: Introduction Dopaminergic neurons in the midbrain show a variety of firing patterns, ranging from very regular firing pacemaker cells to bursty and irregular neurons. The effects of different experimental conditions (like pharmacological treatment or genetical manipulations) on these neuronal discharge patterns may be subtle. Background: Three genes have been confirmed as major joint susceptibility genes for endocrine autoimmune disease:human leukocyte antigen class II, cytotoxic T-lymphocyte antigen 4 and protein tyrosine phosphatase non-receptor type 22. Recent studies showed that a genetic variation within the interferon induced helicase domain 1 (IFIH1) locus (rs1990760 polymorphism) is an additional risk factor in type 1 diabetes and Graves' disease (GD). Methods: The aim of the present study was to investigate the role of the rs1990760 polymorphism within the IFIH1 gene in German patients with GD (n=258), Hashimoto's thyroiditis (HT,n=106), Addison's disease (AD,n=195) and healthy controls (HC,n=227) as well as in 55 GD families (165 individuals, German) and 100 HT families (300 individuals, Italian). Furthermore, the interaction between rs1990760 polymorphism with human leukocyte antigen (HLA) risk haplotype DQ2(DQA*0501-DQB*0201), the risk haplotypes DQ2/DQ8 (DQA*0301-DQB*0302) and the status of thyroglobulin antibody (TgAb), thyroid peroxidase antibody (TPOAb) and TSH receptor antibody (TRAb) in patients and families were analysed. Results:No significant differences were found between the allele and genotype frequencies for rs1990760 IFIH1 polymorphism in patients with GD, HT, AD and HC. Also no differences were observed when stratifying the IFIH1 rs1990760 polymorphism for gender, presence or absence of thyroid antibodies (GD:TRAb and HT:TPOAb/TgAb) and HLA risk haplotypes (DQ2:for GD and HT, DQ2/DQ8:for AD). Furthermore the transmission analysis in GD and HT families revealed no differences in alleles transmission for rs1990760 IFIH1 from parents with or without HLA risk haplotype DQ2 to the affected offspring. In contrast, by dividing the HT parents according to the presence or absence of thyroid Ab titers, mothers and fathers both positive for TPOAb/TgAb overtransmitted the allele A of IFIH1 rs1990760 to their HT affected offspring (61.8% vs 38.2%;p=0.05;corrected p [pc]=0.1). However, these associations did not remain statistically significant after correction of the p-values. Conclusion: In conclusion, our data suggest, no contribution from IFIH1 rs1990760 polymorphism to the pathogenesis of either Graves' disease, Hashimoto's thyroiditis or Addison's disease in our study populations. However, in order to exclude a possible influence of the studied polymorphism in specified subgroups within patients with autoimmune thyroid disease, further investigations in larger populations are needed.
Typ 1 Diabetes mellitus (T1D), Hashimoto-Thyreoiditis (HT) und Morbus Addison (AD) sind autoimmunvermittelte Erkrankungen mit multifaktorieller und polygener Ätiologie. Die gemeinsamen prädisponierenden genetischen Merkmale (HLA Klasse II-Moleküle und CTLA-4) bedingen eine Störungen in der T-Zell-Aktivierung und Homöostase. Es mehren sich Hinweise, dass PTPN22 ein weiterer genereller Risikofaktor für Autoimmunität ist. PTPN22 kodiert für eine intrazelluläre Tyrosin-Phosphatase, die als Suppressor der T-Zell- Aktivierung wirk. In der vorliegenden Arbeit wurde die Verteilung des PTPN22 1858 C/T Polymorphismus in deutschen Patienten mit T1D (n = 220), AD (n = 121) und HT (n = 116) sowie gesunden Kontrollen (n = 239) untersucht. Hierbei konnte eine signifikante Assoziation der Genvariation und den Erkrankungen T1D und HT bestätigt werden. PTPN22 1858 T wurde häufiger bei Patienten mit T1D (19,3% vs. 11,3%; p = 0,001; OR für Allel T = 1,88, 95% CI [1,30-2,72]) und HT (18,1% vs. 11,3%; p = 0,0129; OR für Allel T = 1,74, 95% CI [1,12-2,69]) beobachtet. Erstmals konnte eine exklusive Assoziation zum weiblichen Geschlecht bei Patientinnen mit T1D beobachtet werden (p = 0,0001). Ein signifikanter Unterschied zwischen männlichen Typ 1 Diabetikern und Kontrollen war nicht nachweisbar. Für eine Assoziation von PTPN22 1858 C/T und AD fanden sich keine Hinweise. Zusammenfassend ist der Polymorphismus PTPN22 1858 C/T in der Pathogenese von HT und T1D in der deutschen Bevölkerung involviert, beim T1D durch einen geschlechtsspezifischen Mechanismus, der zu einer Risikoerhöhung im weiblichen Geschlecht führt. Diese Ergebnisse werden durch aktuelle Studien bestätigt. Der Vitamin D-Stoffwechsel ist ein weiteres System mit einem klar belegten Einfluss auf das Immunsystem. Die Gabe von aktivem Vitamin D zeigte im Tiermodell eine Protektion vor T1D. Der modulierende Einfluss auf T-Zell-Differenzierung und Aktivität von dendritischen Zellen mittels Zytokinausschüttung bewirkt ein Verschiebung des Gleichgewichts von T-Helferzellen in Richtung Th2-Zellen und regulatorischen T-Zellen. Zusätzlich belegen epidemiologische Daten eine Verbindung von Vitamin D Ernährungsstatus in der frühen Kindheit und der Prävalenz von T1D. Ein genetisches Risiko für T1D konnte für die folgenden Komponenten des Vitamin D-Systems identifiziert werden: Vitamin D bindendes Protein (DBP), CYP2R1, CYP27B1 und CYP24 sowie Vitamin D-Rezeptor (VDR). Ein weiteres Makromolekül mit essentieller Bedeutung für die Aufrechterhaltung des Vitamin D-Metabolismus ist Megalin. Der endozytotische Multiliganden-Rezeptor gehört zur Familie der low-density Lipoprotein (LDL)-Rezeptoren und wird hauptsächlich in den Zellen des proximalen Tubulus der Niere exprimiert, zusätzlich aber auch in anderen resorptiven Epithelien. Megalin liefert den größten Beitrag zur Protein-Aufnahme aus dem glomerulären Filtrat in die Zellen des proximalen Tubulus. Neben einer relativ unspezifischen Bindung vieler Proteine (z.B.: Albumin, ベ1- und ベ2-Mikroglobulin) bindet Megalin einige Liganden mit hoher Affinität. Hierzu zählen Hormone, Vitamin-bindende Proteine und Lipoproteine. Megalin gewinnt durch die zelluläre Aufnahme von Vitamin D im Komplex mit seinem Transportprotein DBP eine große Bedeutung im Vitamin D-Stoffwechsel. Die Endozytose ist notwendig um 25 OH D3 zu konservieren und um Zellen mit der Vorstufe zur Generierung von 1,25 (OH)2 D3, der biologisch aktiven Form von Vitamin D, zu versorgen. Weiterhin ist die Aufnahme möglicherweise direkt mit der Aktivierung des nukleären VDR verbunden, als Vorbereitung auf kommende Liganden In der vorliegenden Arbeit wurde erstmals die Bedeutung von genetischen Polymorphismen des Megalingens bei Patienten mit T1D und gesunden Kontrollen der deutschen Bevölkerung untersucht. Die Analyse von neun SNPs erbrachten interessante Erkenntnisse: Genvariationen von Megalin sind mit T1D assoziiert, wobei MegE36 A, ein SNP auf Exon 36, das Risiko einer Erkrankungsmanifestation erhöht. Allel »A« wurde häufiger bei Patienten mit T1D beobachtet (95,6% vs. 92,2%; OR für Allel A = 1,85, 95% CI [1,18-2,91]). In der geschlechtsgetrennten Analyse beschränkten sich die gefundenen Differenzen der Allelfrequenzen lediglich auf das männliche Geschlecht (p = 0,0014; OR für Allel A = 2,67, 95% CI [1,43 – 4,98]). Bei Frauen war kein signifikanter Unterschied nachweisbar. Da dieser exonische SNP still ist, d.h. durch einen synonymen Aminosäurenaustausch keinen Einfluss auf die Rezeptorfunktionalität hat, treten mögliche Effekte auf RNA-Ebene auf. Denkbar ist auch eine Kopplung mit einem benachbarten Marker. Drei weitere nicht synonyme exonische SNPs (MegE03 A/G, MegE66 A/G and MegE69 A/C) weisen eine grenzwertige Assoziation zu T1D auf und haben zugleich einen funktionellen Einfluss auf die Proteinstruktur. Bemerkenswerterweise zeigt MegE66, wie auch MegE36 A, lediglich bei Männern ein genetisches Risiko für T1D. Diese Geschlechtsspezifität wird möglicherweise durch eine Interaktion von Megalin mit Androgenen und Östrogenen in Verbindung mit dem Transportprotein (SHBG) begründet. Die Aufklärung des molekularen Mechanismus wird Gegenstand künftiger Studien sein.
Introduction: Reliable predictive and prognostic markers for routine diagnostic purposes are needed for breast cancer patients treated with neoadjuvant chemotherapy. We evaluated protein biomarkers in a cohort of 116 participants of the GeparDuo study on anthracycline/taxane-based neoadjuvant chemotherapy for operable breast cancer to test for associations with pathological complete response (pCR) and disease-free survival (DFS). Particularly, we evaluated if interactions between hormone receptor (HR) and human epidermal growth factor receptor 2 (HER2) expression might lead to a different clinical behavior of HR+/HER2+ coexpressing and HR+/HER2- tumors and whether subgroups of triple negative tumors might be identified by the help of Ki67 labeling index, cytokeratin 5/6 (CK5/6), as well as cyclooxygenase-2 (COX-2), and Y-box binding protein 1 (YB-1) expression. Methods: Expression analysis was performed using immunohistochemistry and silver-enhanced in situ hybridization on tissue microarrays (TMAs) of pretherapeutic core biopsies. Results: pCR rates were significantly different between the biology-based tumor types (P = 0.044) with HR+/HER2+ and HR-/HER2- tumors having higher pCR rates than HR+/HER2-tumors. Ki67 labeling index, confirmed as significant predictor of pCR in the whole cohort (P = 0.001), identified HR-/HER- (triple negative) carcinomas with a higher chance for a pCR (P = 0.006). Biology-based tumor type (P = 0.046 for HR+/HER2+vs. HR+/HER2-), Ki67 labeling index (P = 0.028), and treatment arm (P = 0.036) were independent predictors of pCR in a multivariate model. DFS was different in the biology-based tumor types (P < 0.0001) with HR+/HER2- and HR+/HER2+ tumors having the best prognosis and HR-/HER2+ tumors showing the worst outcome. Biology-based tumor type was an independent prognostic factor for DFS in multivariate analysis (P < 0.001). Conclusions: Our data demonstrate that a biology-based breast cancer classification using estrogen receptor (ER), progesterone receptor (PgR), and HER2 bears independent predictive and prognostic potential. The HR+/HER2+ coexpressing carcinomas emerged as a group of tumors with a good response rate to neoadjuvant chemotherapy and a favorable prognosis. HR+/HER2- tumors had a good prognosis irrespective of a pCR, whereas patients with HR-/HER- and HR-/HER+ tumors, especially if they had not achieved a pCR, had an unfavorable prognosis and are in need of additional treatment options. We report the case of a 62-year-old man with chronic pancreatitis who presented with increasing abdominal pain. Sonography, magnetic resonance imaging, contrast-enhanced computed tomography, and ultimately catheter angiography demonstrated a pancreatic pseudocyst that had eroded into the splenoportal venous confluence, mimicking an arterial aneurysm. The diagnostic was confirmed at the time of surgical treatment. This case demonstrates the use of imaging to diagnose complications of pancreatitis, and the difficulty of distinguishing an eroding pseudocyst from an arterial aneurysm.
Dendritic cells are the sentinels between the innate and the adaptive immunity. They are professionals that capture invading pathogens, recognize specific microbial structures and induce naïve T lymphocytes to polarize into a specific T cell subset. To initiate the T cell polarization DCs secrete cytokines which are induced upon Toll-like receptor activation by microbial structures. The recognition of these structures and the discrimination between non-self and self structures by TLRs is fine tuned, but under defined circumstances deregulation of immune responses appears. Consequently, this can result in immune disorders such as autoimmunity, chronic inflammatory diseases or cancer. In this thesis the investigations are focused on the regulation of the IL-12 family members IL-12p70 and IL-23 in DCs. The objective was to investigate three different endogenous and exogenous factors that regulate IL-12p70 or IL-23. In the first part Selenium, an essential trace element and important factor in several metabolic pathways including the cellular redox status and reactive oxygen species (ROS) dependent signaling was applied as supplement in immature Langerhans cell culture. Because Selenium also plays a role in the immune system the TLR-induced IL-23 production of the DCs upon Selenium treatment was analyzed. In the immature Langerhans cell line XS-52 the strongest inducer of IL-23 was TLR4 ligand LPS. Furthermore increased levels of TLR4-induced IL-23 in cells treated with Selenium were detected in a concentration dependent manner. Whereas the IL-23 subunit p40 was upregulated upon Selenium treatment the second subunit p19 was completely unaffected. This effect was detected on mRNA and protein level. In addition, as expected, IFN-gamma inhibited the TLR4-induced IL-23 secretion of both, Selenium treated and untreated cells. In the second part of this thesis p47phox, an organizing protein of the NADPH oxidase was analyzed regarding its potential to regulate IL-12p70 and/or IL-23 secreted by different DC subtypes. Since it was demonstrated that p47phox deficiency is associated with enhanced autoimmunity and chronic inflammation we wanted to prove whether it has a function in addition to that within the NADPH oxidase. We found some hints that p47phox may be interact with proteins of the TLR signaling pathway and thus we hypothesized that p47phox may have a function for the regulation of TLR-mediated cytokine production in DCs. In several experiments with DCs from the spleen of different p47phox deficient mice we detected an increased production of TLR9-induced IL-12p70 compared to wild type cells. In contrast TLR4 stimulation with LPS displayed no significant differences between p47phox deficient and wild type cells. In spleen cells IL-23 was not detected. Confirming the results of this new negative feedback by p47phox on IL-12p70 rats, with a single nucleotide polymorphism in the p47phox gene, were investigated. Interestingly this polymorphism is located in the phosphorylation site of IRAK4, an important kinase in the TLR pathway. In rats with a methionine residue at this position in the p47phox protein enhanced IL-12p70 level were found, compared to the rats with threonine, which can be phosphorylated by IRAK4. All analyzed mice and rats have defects in the NADPH oxidase function due to a non functional p47phox protein which results in a defective ROS production. To determine whether the observed negative feedback mechanism is connected to the lack of ROS production experiments with gp91phox deficient mice, which also have a defective NADPH oxidase function, were performed. In several experiments the enhanced IL-12p70 production in cells from p47phox deficient mice could be confirmed, but no differences between gp91phox deficient and wild type mice have been observed. In further studies was found that the inhibition of the NADPH oxidase function did not alter the negative feedback on TLR9-induced IL-12p70 secretion by p47phox. Interestingly upon treatment with the inhibitor a feedback mechanism in wild type cells also after TLR4 stimulation was observed. Hence, blocking a ROS-dependent TLR4 pathway by the inhibitor uncovered the LPS induced ROS-independent pathway of the TLR4 signaling. These findings strongly approve a NADPH oxidase/ROS-independent function of p47phox in DCs. Because splenic DCs do not secrete IL-23, in vitro differentiated DCs from the bone marrow were investigated regarding the negative feedback mechanism. In DCs from p47phox deficient mice, differentiated with GM-CSF, the upregulation of IL-12p70 was confirmed, whereas Flt3-L cultured DCs did not display the negative feedback. In contrast to IL-12p70 no difference for the IL-23 production between wild type and p47phox deficient cells has been detected. Thus, we concluded that IL-23 production is not regulated by p47phox. IL-12p70 is the major cytokine in the Th1 polarization whereas IL-23 is important for the maintenance and survival of Th17 cells. To prove whether the regulation of IL-12p70 influences the T cell response immunization experiments closely resembling the classical DTH-like protocols were performed. Groups of p47phox deficient and wild type mice received either PBS, OVA alone or mixed with TLR9 ligand CpG2216 in IFA s.c. to activate and polarize naïve T cells towards Th1 or Th17 cells. After ten days isolated lymph node cells were incubated in an ELISA spot assay with or without OVA and the frequency of IFN-gamma and IL-17 producing T cells was quantified. In vitro recall of OVA immunization of wild type and p47phox deficient mice resulted in an increased IFN-gamma and IL-17 frequency in the p47phox deficient cells. The combination with CpG2216 as adjuvant and inducer of the 3rd signal enhanced the frequency of IFN-gamma and IL-17 producing T cells in wild type mice significantly. However, in p47phox deficient cells the IFN-gamma and IL-17 response, being already detectable without in vitro OVA re-stimulation, was strongly augmented upon OVA restimulation. These findings confirmed our in vitro data for IL-12p70. Hence, the data supports our hypothesis that the p47phox dependent regulation of IL-12p70 and the consequences for the T cell response is an important mechanism to prevent uncontrolled immune responses. In the last part of this thesis the immunomodulatory property of vitamin D3 on the IL-12p70 production of DCs was examined. Since it was shown that VD3 influences the differentiation and maturation of monocytes and DCs, splenic DCs from C57BL/6 and BALB/c mice were investigated regarding their IL-12p70 production after VD3 treatment. Spleen cells, stimulated with LPS or CpG2216, exhibited a decreased IL-12p70 production when treated with VD3 before stimulation phase. In contrast treatment with VD3 only during TLR stimulation had no influence on the IL-12p70 production. Since it was demonstrated that VD3 stimulates the expression of p47phox mRNA cells from p47phox deficient mice were also treated with VD3. In initial experiments only a slight inhibition of IL-12p70 has been detected in p47phox deficient cells compared to the wild type. In summary the thesis displays three different possibilities to influence the TLR-induced cytokine secretion of DCs, although with different intensities and specificities.
