Gutartige Schilddrüsenknoten stellen ein häufiges klinisches Problem dar, bei dem minimalinvasive, thermoablative Therapien wie die Radiofrequenzablation (RFA), als Alternative zur chirurgischen Behandlung, immer relevanter werden. Da es sich bei den Beschwerden oftmals um Symptome handelt, die von der lokalen Raumforderung und Größe abhängen, ist die Reduktion des Knotenvolumens ein zentraler Bestandteil der Therapie. Hierbei stellt die bipolare RFA eine neuartige Technologie zur minimalinvasiven Behandlung von gutartigen Schilddrüsenknoten dar und soll die Nachteile der monopolaren RFA, die das bislang am häufigsten verwendete System darstellt, überwinden. Jedoch gibt es aktuell keine offiziellen Leitlinien bezüglich der zu verwendenden Ablationstechnik. Aus diesem Grund war die Zielrichtung dieser Arbeit, erstmalig die Behandlung gutartiger Schilddrüsenknoten mittels bipolarer RFA in Kombination mit der sogenannten multiple overlapping shot technique („MOST“) anhand der Volumenreduktion nach 3 Monaten zu untersuchen. Hierzu wurden 18 Patienten (4 männliche, 14 weibliche Patienten; mittleres Alter: 50 Jahre, Altersspannweite: 15–72 Jahre) mit insgesamt 20 gutartigen Schilddrüsenknoten (17 hypofunktionelle und 3 hyperfunktionelle Knoten) behandelt und anschließend sonographisch ausgewertet. Mit einer medianen Volumen-reduktion (ΔV) von 5,3 ml (Spannweite: 0,13– 43,1 ml) nach 3 Monaten, was einer mittleren relativen Volumenreduktion von 56 ± 17,9 % entspricht, ergab sich ein signifikantes (p < 0,0001) Ergebnis. Das mediane Knotenvolumen reduzierte sich von anfangs 8 ml (Spannweite: 0,48– 62 ml) auf 2,3 ml (Spannweite: 0,3–32 ml) bei der Kontrolluntersuchung. Es kam bei allen Knoten zu einer Volumenreduktion, welche in 70 % der Fälle über 50 % betrug. Schwere Komplikationen wie anhaltende Stimmveränderungen, Nervenläsionen, Knoten-rupturen und Infektionen, ebenso wie immunogene Überfunktionen oder persistierende Schmerzen, traten während des Follow-ups nicht auf. Alle Patienten mit hypofunktionellen Knoten (15) blieben euthyreot, während sich Patienten mit hyperfunktionellen Knoten zum Zeitpunkt der Kontrolluntersuchung entweder in einer euthyreoten (2) oder latent hypothyreoten (1) Stoffwechsellage befanden. Ein medikationspflichtiger Hypothyreoidismus entstand bei keinem der Patienten. Daher stellt die bipolare RFA in Kombination mit der angewandten MOST ein effektives und sicheres thermoablatives Verfahren für die Behandlung gutartiger Schilddrüsenknoten dar.
Forgetting is a common phenomenon in everyday life. Although it often has negative connotations, forgetting is an important adaptive mechanism to avoid loading the memory storage with irrelevant information. A very important aspect of forgetting is its interaction with emotion. Affective events are often granted special and priority treatment over neutral ones with regards to memory storage. As a consequence, emotional information is more resistant to extinction than neutral information. It has been suggested that intentional forgetting serves as a mechanism to cope with unwanted or disruptive emotional memories and the main goal of this study was to assess forgetting of emotional auditory material using the item-method directed forgetting (DF) paradigm using a forgetting strategy based on mindfulness as a means to enhance DF. Contrary to our prediction, the mindfulness-based strategy not only did not improve DF but reduced it for neutral material. These results suggest that an interaction between processes such as response inhibition and attention is required for intentional forgetting to succeed.
Carcinogenesis is a multistep process. Besides somatic mutations in tumor cells, stroma-associated immunity is a major regulator of tumor growth. Tumor cells produce and secrete diverse mediators to create a local microenvironment that supports their own survival and growth. It is becoming apparent that iron acquisition, storage, and release in tumor cells is different from healthy counterparts. It is also appreciated that macrophages in the tumor microenvironment acquire a tumor-supportive, anti-inflammatory phenotype that promotes tumor cell proliferation, angiogenesis, and metastasis. Apparently, this behavior is attributed, at least in part, to the ability of macrophages to support tumor cells with iron. Polarization of macrophages by apoptotic tumor cells shifts the profile of genes involved in iron metabolism from an iron sequestering to an iron-release phenotype. Iron release from macrophages is supposed to be facilitated by ferroportin. However, lipid mediators such as sphingosine-1-phosphate, released form apoptotic tumor cells, upregulate lipocalin-2 (Lcn-2) in macrophages. This protein is known to bind siderophore-complexed iron and thus, may participate in iron transport in the tumor microenvironment. We describe how macrophages handle iron in the tumor microenvironment, discuss the relevance of an iron-release macrophage phenotype for tumor progression, and propose a new role for Lcn-2 in tumor-associated macrophages.
Exposure to community violence through witnessing or being directly victimized has been associated with conduct problems in a range of studies. However, the relationship between community violence exposure (CVE) and conduct problems has never been studied separately in healthy individuals and individuals with conduct disorder (CD). Therefore, it is not clear whether the association between CVE and conduct problems is due to confounding factors, because those with high conduct problems also tend to live in more violent neighborhoods, i.e., an ecological fallacy. Hence, the aim of the present study was: (1) to investigate whether the association between recent CVE and current conduct problems holds true for healthy controls as well as adolescents with a diagnosis of CD; (2) to examine whether the association is stable in both groups when including effects of aggression subtypes (proactive/reactive aggression), age, gender, site and socioeconomic status (SES); and (3) to test whether proactive or reactive aggression mediate the link between CVE and conduct problems. Data from 1178 children and adolescents (62% female; 44% CD) aged between 9 years and 18 years from seven European countries were analyzed. Conduct problems were assessed using the Kiddie-Schedule of Affective Disorders and Schizophrenia diagnostic interview. Information about CVE and aggression subtypes was obtained using self-report questionnaires (Social and Health Assessment and Reactive-Proactive aggression Questionnaire (RPQ), respectively). The association between witnessing community violence and conduct problems was significant in both groups (adolescents with CD and healthy controls). The association was also stable after examining the mediating effects of aggression subtypes while including moderating effects of age, gender and SES and controlling for effects of site in both groups. There were no clear differences between the groups in the strength of the association between witnessing violence and conduct problems. However, we found evidence for a ceiling effect, i.e., individuals with very high levels of conduct problems could not show a further increase if exposed to CVE and vice versa. Results indicate that there was no evidence for an ecological fallacy being the primary cause of the association, i.e., CVE must be considered a valid risk factor in the etiology of CD.

Adenomyosis or endometriosis genitalis interna is a frequent benign disease of women in fertile age. It causes symptoms like bleeding disorders and dysmenorrhea and seems to have a negative effect on fertility. Adenomyosis can be part of a complex genital and extragenital endometriosis but also can be found as a solitary uterine disease. While peritoneal endometriosis can be easily diagnosed by laparoscopy with subsequent biopsy, the determination of adenomyosis is difficult. In the following literature review, the diagnostic methods clinical history and symptoms, gynecological examination, 2D and 3D transvaginal ultrasound, MRI, hysteroscopy, and laparoscopy will be discussed step by step in order to evaluate their predictive value in the diagnosis of adenomyosis.
Lymphocyte function-associated antigen 1 (LFA-1) affinity and avidity changes have been assumed to mediate adhesion to intercellular adhesion molecule-1 for T-cell conjugation to dendritic cells (DC). Although the T-cell receptor (TCR) and LFA-1 can generate intracellular signals, the immune cell adaptor protein linker for the activation of T cells (LAT) couples the TCR to downstream events. Here, we show that LFA-1 can mediate both adhesion and de-adhesion, dependent on receptor clustering. Although increased affinity mediates adhesion, LFA-1 cross-linking induced the association and activation of the protein-tyrosine kinases FAK1/PYK1 that phosphorylated LAT selectively on a single Y-171 site for the binding to adaptor complex GRB-2-SKAP1. LAT-GRB2-SKAP1 complexes were distinct from canonical LAT-GADs-SLP-76 complexes. LFA-1 cross-linking increased the presence of LAT-GRB2-SKAP1 complexes relative to LAT-GADs-SLP-76 complexes. LFA-1-FAK1 decreased T-cell-dendritic cell (DC) dwell times dependent on LAT-Y171, leading to reduced DO11.10 T cell binding to DCs and proliferation to OVA peptide. Overall, our findings outline a new model for LFA-1 in which the integrin can mediate both adhesion and de-adhesion events dependent on receptor cross-linking.
