Nonerythroid spectrin αII (SPTAN1) is an important cytoskeletal protein that ensures vital cellular properties including polarity and cell stabilization. In addition, it is involved in cell adhesion, cell-cell contact, and apoptosis. The detection of altered expression of SPTAN1 in tumors indicates that SPTAN1 might be involved in the development and progression of cancer. SPTAN1 has been described in cancer and therapy response and proposed as a potential marker protein for neoplasia, tumor aggressiveness, and therapeutic efficiency. On one hand, the existing data suggest that overexpression of SPTAN1 in tumor cells reflects neoplastic and tumor promoting activity. On the other hand, nuclear SPTAN1 can have tumor suppressing effects by enabling DNA repair through interaction with DNA repair proteins. Moreover, SPTAN1 cleavage products occur during apoptosis and could serve as markers for the efficacy of cancer therapy. Due to SPTAN1’s multifaceted functions and its role in adhesion and migration, SPTAN1 can influence tumor growth and progression in both positive and negative directions depending on its specific regulation. This review summarizes the current knowledge on SPTAN1 in cancer and depicts several mechanisms by which SPTAN1 could impact tumor development and aggressiveness.
Introduction: Colorectal cancers (CRCs) deficient in the DNA mismatch repair protein MutL homolog 1 (MLH1) display distinct clinicopathological features and require a different therapeutic approach compared to CRCs with MLH1 proficiency. However, the molecular basis of this fundamental difference remains elusive. Here, we report that MLH1-deficient CRCs exhibit reduced levels of the cytoskeletal scaffolding protein non-erythroid spectrin αII (SPTAN1), and that tumor progression and metastasis of CRCs correlate with SPTAN1 levels.
Methods and results: To investigate the link between MLH1 and SPTAN1 in cancer progression, a cohort of 189 patients with CRC was analyzed by immunohistochemistry. Compared with the surrounding normal mucosa, SPTAN1 expression was reduced in MLH1-deficient CRCs, whereas MLH1-proficient CRCs showed a significant upregulation of SPTAN1. Overall, we identified a strong correlation between MLH1 status and SPTAN1 expression. When comparing TNM classification and SPTAN1 levels, we found higher SPTAN1 levels in stage I CRCs, while stages II to IV showed a gradual reduction of SPTAN1 expression. In addition, SPTAN1 expression was lower in metastatic compared with non-metastatic CRCs. Knockdown of SPTAN1 in CRC cell lines demonstrated decreased cell viability, impaired cellular mobility and reduced cell-cell contact formation, indicating that SPTAN1 plays an important role in cell growth and cell attachment. The observed weakened cell-cell contact of SPTAN1 knockdown cells might indicate that tumor cells expressing low levels of SPTAN1 detach from their primary tumor and metastasize more easily.
Conclusion: Taken together, we demonstrate that MLH1 deficiency, low SPTAN1 expression, and tumor progression and metastasis are in close relation. We conclude that SPTAN1 is a candidate molecule explaining the tumor progression and metastasis of MLH1-deficient CRCs. The detailed analysis of SPTAN1 is now mandatory to substantiate its relevance and its potential value as a candidate protein for targeted therapy, and as a predictive marker of cancer aggressiveness.
Poster presentation: The mammalian pineal organ is a peripheral oscillator, depending on afferent information from the so-called master clock in the suprachiasmatic nuclei of the hypothalamus. One of the best studied outputs of the pineal gland is the small and hydrophobic molecule melatonin. In all vertebrates, melatonin is synthesized rhythmically with high levels at night, signalling the body the duration of the dark period. Changes or disruptions of melatonin rhythms in humans are related to a number of pathophysiological disorders, like Alzheimer's disease, seasonal affective disorder or the Smith-Magenis-Syndrome. To use melatonin in preventive or curative interferences with the human circadian system, a complete understanding of the generation of the rhythmic melatonin signal in the human pineal gland is essential. Melatonin biosynthesis is best studied in the rodent pineal gland, where the activity of the penultimate and rate-limiting enzyme, the arylalkylamine N-acetyltransferase (AA-NAT), is regulated on the transcriptional level, whereas the regulatory role of the ultimate enzymatic step, achieved by the hydroxyindole O-methyltransferase (HIOMT), is still under debate. In rodents, Aa-nat mRNA is about 100-fold elevated during the night in response to adrenergic stimulation of the cAMP-signalling pathway, with AA-NAT protein levels closely following this dynamics. In contrast, in all ungulates studied so far (cow, sheep), a post-transcriptional regulation of the AA-NAT is central to determine rhythmic melatonin synthesis. AA-NAT mRNA levels are constantly elevated, and lead to a constitutive up-regulation of AA-NAT protein, which is, however, rapidly degraded via proteasomal proteolysis during the day. AA-NAT proteolysis is only terminated upon the nocturnal increase in cAMP levels. Similar to ungulates, a post-transcriptional control of this enzyme seems evident in the pineal gland of the primate Macaca mulatta. Studies on the molecular basis of melatonin synthesis in the human being are sparse and almost exclusively based on phenomenological data, derived from non-invasive investigations. Yet the molecular mechanisms underlying the generation of the hormonal message of darkness can currently only be deciphered using autoptic material. We therefore analyzed in human post-mortem pineal tissue Aa-nat and Hiomt mRNA levels, AA-NAT and HIOMT enzyme activity, and melatonin levels for the first time simultaneously within tissue samples of the same specimen. Here presented data show the feasibility of this approach. Our results depict a clear diurnal rhythm in AA-NAT activity and melatonin content, despite constant values for Aa-nat and Hiomt mRNA, and for HIOMT activity. Notably, the here elevated AA-NAT activity during the dusk period does not correspond to a simultaneous elevation in melatonin content. It is currently unclear whether this finding may suggest a more important role of the ultimate enzyme in melatonin synthesis, the HIOMT, for rate-limiting the melatonin rhythm, as reported recently for the rodent pineal gland. Thus, our data support for the first time experimentally that post-transcriptional mechanisms are responsible for the generation of rhythmic melatonin synthesis in the human pineal gland.
Patienten mit akuter Dekompensation einer Leberzirrhose (AD) oder einem akut-auf-chronischen-Leberversagen (ACLF) stellen ein vulnerables Kollektiv für den Erhalt eines Erythrozytenkonzentrates (EK) dar. Zu den Ursachen zählen das häufige Auftreten einer gastrointestinalen Blutung, die Koagulopathie oder das Vorliegen einer chronischen Anämie. Während für viele andere Patientenkollektive das richtige Transfusionsmanagement bereits erforscht worden ist, fehlen diese Studien für Patienten mit Leberzirrhose, insbesondere für die neue Entität ACLF. Die vorliegende Studie soll die Auswirkung einer EK-Transfusion auf Morbidität und Mortalität dieser Patienten untersuchen.
Insgesamt wurden 498 Patienten mit der Diagnose einer Leberzirrhose, die zwischen den Jahren 2015 und 2019 auf eine Intensivstation der Universitätsklinik Frankfurt aufgenommen worden sind, retrospektiv analysiert. Für die statistische Auswertung wurde ein Prospensity-Score-Matching nach EK-Transfusion mit Adjustierung für mögliche Konfundierungseffekte durchgeführt. Der Einfluss der Transfusion auf die Mortalität wurde mithilfe von Kaplan-Meier-Kurven und multivariater Cox-Regression untersucht. Für die ACLF-Kohorte wurden ROC-Kurven zum Versuch der Identifizierung eines Transfusionstriggers und eines Zielhämoglobinwertes nach Transfusion angefertigt.
In der Gesamtkohorte wiesen transfundierte Patienten eine signifikant höhere Mortalitätsrate als Nicht-Transfundierte auf (28-Tages-Mortalität: 39,6% vs. 19,5%, p<0,001). Dabei wirkte sich die Transfusion primär bei Patienten mit ACLF negativ auf das Überleben aus. Nach Matching der Patienten nach Erhalt eines EKs und Adjustierung für potentielle Konfundierungseffekte blieb die Kurzzeitmortalität bei transfundierten Patienten mit ACLF weiterhin signifikant erhöht (28-Tages-Mortalität: 72,7% vs. 45,5%, p=0,03). Bei AD Patienten zeigte die Transfusion keinen signifikanten Einfluss auf das Überleben. Die multivariate Cox-Regression identifizierte die EK-Transfusion als unabhängigen Risikofaktor der Kurzzeitmortalität im ACLF (HR: 2,55; 95% KI 1,26 – 5,15, p=0,009). Die Identifizierung eines Transfusionstriggers oder eines Zielhämoglobinwertes war nicht möglich.
Die vorliegende Studie konnte zeigen, dass die EK-Transfusion einen Risikofaktor der Kurzzeitmortalität im ACLF, aber nicht für AD Patienten darstellt. Dieses Ergebnis macht fortführende Untersuchungen zur Ursachenforschung und zur Evaluierung des richtigen Transfusionsmanagements für Patienten mit ACLF notwendig.
Die Schizophrenie ist eine der wesentlichen psychiatrischen Erkrankungen. Obschon seit langem bekannt, bleibt insbesondere die Schizophrenie trotz zahlreicher und aufwändiger Forschungsansätze hinsichtlich ihrer vielfältigen Erscheinungsmöglichkeiten bislang in ihrem Grunde unverstanden und rätselhaft. Neuere Studien der letzten Jahre lenkten das Interesse u.a. auf die Region des präfrontalen Cortex, dessen Grundfunktionen durch die Schizophrenie besonders alteriert zu sein scheinen. Morphometrische Arbeiten ergaben Hinweise für eine Reduktion des Neuropils in diesem Gebiet. Diesem zugrunde liegend wird eine Reduktion der dendritischen Substanz diskutiert. Diesen Hinweisen für eine Konnektivitätsalteration ist die vorliegende Studie nachgegangen. Es wurde die bislang kaum derartig untersuchte Area 9, als bedeutende und zentrale Region des präfrontalen Cortex, aus einem Kollektiv von 10 Schizophrenen und 8 Kontrollen für jeweils beide Hemisphären präpariert und mittels der Golgi-Methode dargestellt. Der Fokus lag hierbei auf dem basalen Dendritenbaum pyramidaler Neurone der Lamina III und V. Pro Lamina wurden 10 Zellen erfasst und damit pro Fall 40 Zellen (2 Laminae pro Hemisphäre) dargestellt. Insgesamt konnten so 720 Zellen graphisch dargestellt werden (400 Zellen bei Schizophrenen und 320 bei Kontrollen). Der basale Dendritenbaum dieser Zellen wurde morphometrisch mittels der Analyse nach Sholl, der Analyse der Dendritengenerationen und mittels eines Spannbreitenvergleichs ausgewertet. Die Ergebnisse der Sholl-Analyse und der Analyse der Dendritengenerationen bezogen auf die Lamina III beider Hemisphären ergeben eine in den mittleren und späteren Verzweigungsabständen bzw. Generationen nahezu durchgängige Tendenz erhöhter dendritischer Parameter des schizophrenen Kollektivs im Vergleich zu dem der Kontrollen. Statistische Signifikanz erlangt diese Tendenz in Bezug auf die Lamina III der rechten Hemisphäre sowohl in der IV. Generation (p = 0,03) als auch in einem Verzweigungsabstand von 100 μm (p = 0,01). Zudem ergibt sich in dem zusätzlich durchgeführten Spannbreitenvergleich erneut für die Lamina III eine fast durchgehend erhöhte Spannbreite des schizophrenen Kollektivs. In Lamina V dagegen ergibt sich ein weitgehend homogenes Bild beider Kollektive in allen verwandten Analysen. Die Annahme einer Reduktion des Neuropils basierend auf einer Verminderung der dendritischen Substanz im präfrontalen Cortex Schizophrener wird daher durch diese Studie der Lamina III und V der Area 9 und die hierbei angewandten Analysen nicht bestätigt. Dagegen finden sich bezogen auf die Lamina III der Area 9 morphometrisch Hinweise für eine vermehrte dendritische Substanz und eine heterogenere dendritische Architektur im schizophrenen Kollektiv. Diese Beobachtungen können einen wichtigen zusätzlichen Beitrag zu der bei dieser Erkrankung pathogenetisch diskutierten Konnektivitätsalteration darstellen. Als Erklärungsansätze dieser Ergebnisse bieten sich eine Rückbildungsstörung der embryonalen Dendritogenese, ein alterierter Subtyp pyramidaler Neurone der Lamina III oder eine unterschiedliche Ausprägung dieser Befunde bei den verschiedenen Subtypen der Schizophrenie an. Eine definitive Zuordnung der Ergebnisse zu einer der genannten Möglichkeiten ist anhand der Daten dieser Arbeit und des gegenwärtigen Standes der wissenschaftlichen Literatur nicht abschließend möglich. Diese Beobachtungen und die hiermit aufgeworfenen Fragen sind daher im Rahmen zukünftiger Studien zu validieren.
Background: Denosumab treatment for up to 8 years in the FREEDOM study and Extension was associated with low fracture incidence. It was not clear whether subjects who discontinued during the study conduct had a higher risk of fracture than those who remained enrolled, thereby underestimating the true fracture risk for the entire trial cohort. Thus, we explored the influence of early withdrawals on nonvertebral fracture incidence during the Extension study.
Methods: To understand the potential effect of depletion of susceptible subjects on fracture incidence, we first evaluated subject characteristics in patients who were enrolled in the Extension vs those who were not. We subsequently employed a Kaplan-Meier multiple imputation (KMMI) approach to consider subjects who discontinued as if they remained enrolled with a 0%, 20%, 50%, and 100% increase in fracture risk compared with participants remaining on study.
Results: Extension enrollees were generally similar to nonparticipants in median age (71.9 and 73.1 years, respectively), mean total hip bone mineral density T-score (–1.9 and –2.0, respectively), and probability of fracture risk by Fracture Risk Assessment Tool (FRAX®) at FREEDOM baseline (16.9% and 17.7% for major osteoporotic fracture and 6.7% and 7.4% for hip fracture, respectively). When we assumed a doubled fracture risk (100% increase) after discontinuation in KMMI analyses, nonvertebral fracture rate estimates were only marginally higher than the observed rates for both the crossover group (10.32% vs 9.16%, respectively) and the long-term group (7.63% vs 6.63%, respectively).
Conclusion: The observation of continued denosumab efficacy over 8 years of treatment was robust and does not seem to be explained by depletion of susceptible subjects.

Background: Plasma transfusions are most commonly used therapeutically for bleeding or prophylactically in non-bleeding patients prior to invasive procedures or surgery. Although plasma transfusions generally seem to decline, plasma usage for indications that lack evidence of efficacy prevail. Summary: There is wide international, interinstitutional, and interindividual variance regarding the compliance with guidelines based on published references, supported by appropriate testing. There is furthermore a profound lack of evidence from randomized controlled trials comparing the effect of plasma transfusion with that of other therapeutic interventions for most indications, including massive bleeding. The expected benefit of a plasma transfusion needs to be balanced carefully against the associated risk of adverse events. In light of the heterogeneous nature of bleeding conditions and their rapid evolvement over time, fibrinogen and factor concentrate therapy, directed at specific phases of coagulation identified by alternative laboratory assays, may offer advantages over conventional blood product ratio-driven resuscitation. However, their outcome benefit has not been demonstrated in well-powered prospective trials. This systematic review will detail the current evidence base for plasma transfusion in adult surgical patients.
Cholinesterase alterations in delirium after cardiosurgery: a German monocentric prospective study

Objectives: Postoperative delirium (POD) is a common complication after elective cardiac surgery. Recent evidence indicates that a disruption in the normal activity of the cholinergic system may be associated with delirium.
Design: Prospective observational study.
Setting: Single-centre at a European academic hospital.
Primary: and secondary outcome measures In our study the enzyme activities of acetylcholinesterase (AChE) and butyrylcholinesterase (BChE) were determined preoperatively as well as on the first and second postoperative day. The confusion assessment method for the intensive care unit was used to screen patients for the presence of POD.
Results: A total of 114 patients were included in the study. POD was associated with a decrease in BChE activity on postoperative day 1 (p=0.03). In addition, patients who developed POD, had significantly lower preoperative AChE activity than patients without POD (p<0.01). Multivariate analysis identified a preoperatively decreased AChE activity (OR 3.1; 95% CI 1.14 to 8.46), anticholinergic treatment (OR 5.09; 95% CI 1.51 to 17.23), elevated European System for Cardiac Operative Risk Evaluation (OR 3.68; 95% CI 1.04 to 12.99) and age (OR 3.02; 95% CI 1.06 to 8.62) to be independently associated with the development of POD.
Conclusions: We conclude that a reduction in the acetylcholine hydrolysing enzyme activity in patients undergoing cardiac surgery may correlate with the development of POD.
Background: Treatment options for poorly differentiated (PDTC) and anaplastic (ATC) thyroid carcinoma are unsatisfactory and prognosis is generally poor. Lenvatinib (LEN), a multi-tyrosine kinase inhibitor targeting fibroblast growth factor receptors (FGFR) 1-4 is approved for advanced radioiodine refractory thyroid carcinoma, but response to single agent is poor in ATC. Recent reports of combining LEN with PD-1 inhibitor pembrolizumab (PEM) are promising. Materials and Methods: Primary ATC (n=93) and PDTC (n=47) tissue samples diagnosed 1997-2019 at five German tertiary care centers were assessed for PD-L1 expression by immunohistochemistry using Tumor Proportion Score (TPS). FGFR 1-4 mRNA was quantified in 31 ATC and 14 PDTC with RNAscope in-situ hybridization. Normal thyroid tissue (NT) and papillary thyroid carcinoma (PTC) served as controls. Disease specific survival (DSS) was the primary outcome variable. Results: PD-L1 TPS≥50% was observed in 42% of ATC and 26% of PDTC specimens. Mean PD-L1 expression was significantly higher in ATC (TPS 30%) than in PDTC (5%; p<0.01) and NT (0%, p<0.001). 53% of PDTC samples had PD-L1 expression ≤5%. FGFR mRNA expression was generally low in all samples but combined FGFR1-4 expression was significantly higher in PDTC and ATC compared to NT (each p<0.001). No impact of PD-L1 and FGFR 1-4 expression was observed on DSS. Conclusion: High tumoral expression of PD-L1 in a large proportion of ATCs and a subgroup of PDTCs provides a rationale for immune checkpoint inhibition. FGFR expression is low thyroid tumor cells. The clinically observed synergism of PEM with LEN may be caused by immune modulation.
MutLα ist Bestandteil des Mismatch-Reparatur-Systems und spielt eine wichtige
Rolle bei der postreplikativen Reparatur von Kopierfehlern, der Detektion von DNASchäden
durch exogene Noxen und der Signalisierung von irreparablen DNALäsionen
an die Apoptosemaschinerie. MutLα setzt sich als Heterodimer aus MLH1
und PMS2 zusammen. Da sein Fehlen zur Entstehung von Krebs führt, werden
MLH1 und PMS2 zu den Tumorsuppressorproteinen gezählt.
Von vielen krebsassoziierten Proteinen, darunter p53, BRCA und c-Abl, ist bereits
bekannt, dass sie zwischen nukleärer und zytoplasmatischer Lokalisation wechseln.
Dagegen wurde MutLα in der Vergangenheit vorrangig in nukleärer Funktion und
Lokalisation wahrgenommen. Jedoch haben Brieger et al. kürzlich zahlreiche
Interaktionen mit zytoplasmatischen Proteinen aufgedeckt, was nahe legt, dass
MutLα möglicherweise auch wichtige zytosolische Aufgaben hat (Brieger et al.
2010a). Während der Import von MutLα in den Kern bereits aufgeklärt ist, gibt es
über den Export ins Zytosol bislang nur vage Kenntnisse. Hauptfokus dieser Arbeit
ist es deshalb, MutLα auf seine Fähigkeit zum nukleären Export zu untersuchen.
Wir konnten mithilfe eines von Henderson und Eleftheriou entwickelten in vitro
Export-Assays (Henderson, Eleftheriou 2000) zeigen, dass MutLα eine aktive
nukleäre Export-Sequenz im Bereich der Aminosäuren 578-595 von MLH1 besitzt.
Die gezielte Mutation von Leucinen in diesem Bereich veränderte die subzelluläre
Lokalisation von MutLα. Auch setzten solche Mutationen häufig die Stabilität und
die Mismatch-Reparatur-Aktivität des Proteins herab.
Die Untersuchung einer von Han et al. als pathogen beschriebenen Mutation in
diesem Bereich des MLH1-Gens, MLH1L582V (Han et al. 1995), zeigte, dass der
Defekt weder die Proteinstabilität noch die Reparatureigenschaft von MutLα
beeinträchtigte. Jedoch wies das veränderte Heterodimer eine eingeschränkte
Exportfähigkeit auf, sodass dieser Funktionsverlust ursächlich für die
Krebserkrankung des Mutationsträgers sein könnte.
In ∼30% of families affected by colorectal adenomatous polyposis, no germline mutations have been identified in the previously implicated genes APC, MUTYH, POLE, POLD1, and NTHL1, although a hereditary etiology is likely. To uncover further genes with high-penetrance causative mutations, we performed exome sequencing of leukocyte DNA from 102 unrelated individuals with unexplained adenomatous polyposis. We identified two unrelated individuals with differing compound-heterozygous loss-of-function (LoF) germline mutations in the mismatch-repair gene MSH3. The impact of the MSH3 mutations (c.1148delA, c.2319−1G>A, c.2760delC, and c.3001−2A>C) was indicated at the RNA and protein levels. Analysis of the diseased individuals’ tumor tissue demonstrated high microsatellite instability of di- and tetranucleotides (EMAST), and immunohistochemical staining illustrated a complete loss of nuclear MSH3 in normal and tumor tissue, confirming the LoF effect and causal relevance of the mutations. The pedigrees, genotypes, and frequency of MSH3 mutations in the general population are consistent with an autosomal-recessive mode of inheritance. Both index persons have an affected sibling carrying the same mutations. The tumor spectrum in these four persons comprised colorectal and duodenal adenomas, colorectal cancer, gastric cancer, and an early-onset astrocytoma. Additionally, we detected one unrelated individual with biallelic PMS2 germline mutations, representing constitutional mismatch-repair deficiency. Potentially causative variants in 14 more candidate genes identified in 26 other individuals require further workup. In the present study, we identified biallelic germline MSH3 mutations in individuals with a suspected hereditary tumor syndrome. Our data suggest that MSH3 mutations represent an additional recessive subtype of colorectal adenomatous polyposis.
Die gerüstlose aortale Bioprothese 3F nach 5 Jahren
Hintergrund: Die aortale Bioprothese 3F ist eine neue gerüstlose biologische Herzklappe, die aus drei gleichgroßen Segeln aus Pferdepericard besteht, welche in einer tubulären Form zusammengesetzt sind. Diese wird in die native Aortenwurzel implantiert, um somit die erkrankten Segel des Patienten zu ersetzen. Ziel dieser Studie ist es, die Leistungsfähigkeit dieser Erfindung zu beurteilen.
Methoden: In dem Zeitraum zwischen Januar 2002 und September 2004 wurden in der Klinik für Thorax-, Herz- und Gefäßchirurgie der Johann Wolfgang Goethe Universität Frankfurt 47 3F aortale Bioprothesen implantiert. Klinische Ergebnisse wie effektive Öffnungsfläche, Hauptgradienten und Auswurffraktion wurden bei der Entlassung der Patienten, nach 6 Monaten und danach jährlich beurteilt. Die Hauptnachuntersuchung war nach 5,2 ± 1,2 Jahren.
Ergebnisse: Die Nachuntersuchung wurde bei allen Patienten abgeschlossen. Die 30 Tages Sterblichkeit lag bei 2% (n=1). Die späte Sterblichkeit lag bei 22% (Herzbezogen n=4, nicht herzbezogen n= 8). 3 Patienten (6%) entwickelten eine Endokarditis, bei weiteren 2 war eine Reoperation nötig. 4 Patienten (8%) entwickelten ein paravalvuläres Leck, 6 Patienten (12%) hatten einen postoperativen Schlaganfall (4 mit Neueintritt von Vorhofflimmern), 5 Patienten (10%) benötigten eine Rethorakotomie aufgrund von Blutungen. In der Mitte der Laufzeit zeigte die 3F Bioprothese eine gute Hämodynamik mit einem signifikanten Fallen der Durchschnittsgradienten zu 14,5 ± 8,3 mmHg, eine durchschnittliche Öffnungsfläche von 1.4 ± 0,5 cm² und eine durchschnittliche Auswurffraktion von 65 ± 1,4 %. 4 Patienten waren in der NYHA Klasse II, alle anderen in NYHA I in der Mitte der Laufzeit.
Fazit: Die klinische Darbietung der neuen aortalen Bioprothese 3F ist vergleichbar mit regulären gerüstlosen Aortenklappen. Die einzigartigen Konstruktionsmerkmale machen die Implantation leichter und schneller im Vergleich zu konventionellen gerüstlosen Klappen.
Hintergrund: Als Komplikation einer Subarachnoidalblutung können zerebrale Infarkte auftreten. Arterielle Vasospasmen stehen im Verdacht diese auszulösen, jedoch wird zunehmend eine multifaktorielle Ätiologie diskutiert. Die orale Gabe des Calciumantagonisten Nimodipin als Prophylaxe stellt den einzigen pharmakologischen Therapieansatz mit nachgewiesener positiver Wirkung auf das Patientenoutcome dar. Durch die gute Wirksamkeit bei oraler Aufnahme erfolgt die intraarteielle Applikation in von Vasospasmen betroffene Gefäße. Weiterhin wird die Perkutane Transluminale Angioplastie bei fokalen hochgradigen Vasospasmen eingesetzt. Diese endovaskulären Verfahren sind bisher nicht in signifikanten randomisierten Studien untersucht worden.
Zielsetzung: Neu aufgetretene Infarktmuster von Patienten nach SAB wurden hinsichtlich Wirkung und möglicher Komplikationen dieser Therapieansätze untersucht.
Methoden: Die klinisch erfassten Daten und die Bildgebung der im Zeitraum von 01.01.2007 bis 31.12.2011 in der Neuroradiologie der Universitätsklinik Frankfurt am Main behandelten und in die Untersuchung eingeschlossenen 88 Patienten wurde erneut untersucht. Im Falle neu aufgetretener zerebraler Infarkte wurden die Infarktmuster hinsichtlich einer Ätiologie analysiert.
Ergebnisse: 53,4% der Patienten entwickelten nach SAB neue zerebrale Infarkte nach intraarterielle Nimodipinapplikation und zusätzlicher PTA in einzelnen Fällen. Hiervon konnten 89,4% ätiologisch zerebralen Vasospasmen zugeordnet werden. Bei 5,7% aller Patienten traten Infarkte im Zusammenhang mit Komplikationen der intraarteriellen Nimodipingabe und PTA in Form von Thrombembolien und einer Gefäßdissektion auf. Die Ätiologie eines Infarktmusters verblieb unklar.
Fazit: Es konnte kein Vorteil für die intraarterielle Gabe von Nimodipin und PTA bei refraktärem Vasospasmus gezeigt werden. Weiterhin traten in 5,7% des Patientenkollektivs ischämische Komplikationen auf, sodass beide Verfahren nicht als Standardtherapie bei Patienten mit Vasospasmus nach SAB empfohlen werden können.
Die Thrombozytenaktivierung, die PLA-Bildung via CD62, die Leukozytenaktivierung und die entzündliche Aktivität am Endothel nehmen in der heutigen pathophysiologischen Vorstellung der peripheren Atherosklerose und ihrer Entstehung eine zentrale Rolle ein. In dieser Querschnittstudie wurden die PLA-Bildung und Marker der Plättchen-, Leukozyten- und Endothelaktivierung (PAC-1, CD62, Mac-1 und sICAM-1) bei 44 Patienten mit peripherer arterieller Verschlusskrankheit unter Therapie mit ASS (n=17), Clopidogrel (n=12), ihrer Kombination (n=8) oder ohne Therapie (n=7) und bei einer Kontrollgruppe, bestehend aus gesunden Probanden (n=9), untersucht. Die Messungen wurden mittels Flowzytometrie im Vollblut ohne (baseline) und nach in vitro Stimulation mit ADP oder TRAP und eines Immunoassays durchgeführt. Die CD62-Expression zeigte sich bei unbehandelten und mit ASS behandelten Patienten ohne und nach Stimulation signifikant höher als bei mit Clopidogrel oder mit einer Kombination aus ASS und Clopidogrel behandelten Patienten. Gleiches galt für die PLA-Bildung (Monozyten-Leukozyten-Aggregate). Die Mac-1-Expression zeigte sich in den mit TRAP oder ADP stimulierten Proben unter einer Kombina-tionstherapie aus ASS und Clopidogrel gegenüber Patienten ohne oder mit ASS Monotherapie signifikant reduziert. Die sICAM-1-Plasmakonzentrationen waren bei gesunden Probanden und bei antiaggregatorische behandelten Patienten signifikant niedriger als bei unbehandelten Patienten. Die Werte unterschieden sich dabei zwischen den verschiedenen antiaggregatorischen Therapien nur unwesentlich. Dies ist die erste Studie, die zeigt, dass die PLA-Bildung bei Patienten mit pAVK vermehrt ist und unter einer Therapie mit Clopidogrel gemindert wird.
Die natürliche Killerzelllinie NK-92 zeichnet sich durch eine breit gefächerte Aktivität gegen verschiedenste Tumore und Leukämien aus und würde sich daher prinzipiell für eine Verwendung als adoptives Zelltherapeutikum eignen. NK-92-Zellen sind eine von nur 5 etablierten NK-Zelllinien weltweit. Ihr Wachstum in der Zellkultur war bisher von Bedingungen abhängig, die mit einer klinischen Anwendung der Zellen nicht zu vereinbaren sind. Ziel der vorliegenden Doktorarbeit war es daher, ein Kulturverfahren zu etablieren, mit dem sich NK-92-Zellen unter Bedingungen einer „Guten Herstellungs Praxis“ kultivieren und expandieren lassen. In dieser Arbeit wurde daher die Adaption der NK-92-Zellen an ein in der Klinik einsetzbares Zellkulturmedium vorgenommen und ein Batch-Kulturverfahren entwickelt, mit dem sich die NK-92-Zellen innerhalb von 10-14 Tagen auf bis zu 1010 Zellen in 10L Kulturvolumen expandieren lassen. Die Funktionsprüfung der NK-92-Zellen, anhand der Expression von immunologisch relevanten Oberflächenrezeptoren (CD11a, CD25, CD28, CD54, CD56, CD122, FAS-L), ergab keine Veränderung des Phänotyps der expandierten Zellen. Darüber hinaus wiesen die Zellen eine Viabilität von >95,3% +/- 0,46% auf, und ihre zytotoxische Aktivität gegen die NK-sensitive Leukämiezelllinie K562 war nicht eingeschränkt. Da NK-92-Zellen in der Erkennung virusinfizierter und maligner Zellen nicht MHCrestringiert sind, eignen sie sich auch für den ungerichteten Einsatz. Hierzu wäre eine Expansion der Zellen im großen Massstab mit anschliessender Kryokonservierung von Vorteil, da die Zellen dann im Voraus hergestellt und geprüft werden könnten. Die Prüfung des Einflusses unterschiedlicher Konzentrationen (0, 0,5, 1, 2, 3, 5, 8 10 %) der Einfrierschutzlösung Dimethylhylsulfoxid (DMSO) auf die zytotoxische Aktivität der NK-92-Zellen ergab keine Einschränkung der NKZellfunktion bei Konzentrationen < 5%. Es wurden daraufhin verschiedene Einfrierprotokolle und deren Einfluss auf die Viabilität der NK-92-Zellen untersucht. NK-92- Zellen wurden mit 2, 3, 5 8 und 10% DMSO in humanem Serum Albumin (HSA) in Ampullen, oder aber im klinischen Masstab (5x108 Zellen/ 20ml HSA) mit 3, 5 und 10% DMSO eingefroren und ihre Viabilität nach dem Auftauen untersucht. Im Mittel ergab sich für alle Zellpräparationen und DMSO Konzentrationen eine relativ geringe Viabilität der Zellen nach dem Auftauen (<50% +/- 9,77). Hierbei war es unerheblich, ob die für eine klinische Anwendung der allogenen NK-92-Zellen notwendige Bestrahlung mit 10GY vor dem Einfrieren oder nach dem Auftauen durchgeführt wurde (Viabilität 48,8% versus 44%). Aus den in dieser Dissertation erarbeiteten Daten wurde schliesslich ein Konzept zur Expansion der NK-92-Zellen entwickelt, welches ihren klinischen Einsatz, unter Erhalt der Funktionalität bei höchstmöglicher Sicherheit für den Patienten, erlaubt. Dieses Konzept geht von einer Expansion der NK-92-Zellen, ausgehend von einer Masterzellbank, in 2L Batchkulturen im Nunc-Wannenstapel-System aus. Die Kulturen werden mit 2x104 NK-92-Zellen/ml X-Vivo 10 Medium, 5% hitzeinaktiviertem humanen Plasma und 100IE IL-2 beimpft. Nach 10 Tagen haben die Kulturen ihre höchste Dichte (6,4 x105/ml) erreicht.
Previous studies in patients with single-sided deafness (SSD) have reported results of pitch comparisons between electric stimulation of their cochlear implant (CI) and acoustic stimulation presented to their near-normal hearing contralateral ear. These comparisons typically used sinusoids, although the percept elicited by electric stimulation may be closer to a wideband stimulus. Furthermore, it has been shown that pitch comparisons between sounds with different timbres is a difficult task and subjected to various types of range biases. The present study aims to introduce a method to minimize non-sensory biases, and to investigate the effect of different acoustic stimulus types on the frequency and variability of the electric-acoustic pitch matches. Pitch matches were collected from 13 CI users with SSD using the binary search procedure. Electric stimulation was presented at either an apical or a middle electrode position, at a rate of 800 pps. Acoustic stimulus types were sinusoids (SINE), 1/3-octave wide narrow bands of Gaussian noises (NBN), or 1/3-octave wide pulse spreading harmonic complexes (PSHC). On the one hand, NBN and PSHC are presumed to better mimic the spread of excitation produced by a single-electrode stimulation than SINE. On the other hand, SINE and PSHC contain less inherent fluctuations than NBN and may therefore provide a temporal pattern closer to that produced by a constant-amplitude electric pulse train. Analysis of mean pitch match variance showed no differences between stimulus types. However, mean pitch matches showed effects of electrode position and stimulus type, with the middle electrode always matched to a higher frequency than the apical one (p < 0.001), and significantly higher across-subject pitch matches for PSHC compared with SINE (p = 0.017). Mean pitch matches for all stimulus types were better predicted by place-dependent characteristic frequencies (CFs) based on an organ of Corti map compared with a spiral ganglion map. CF predictions were closest to pitch matches with SINE for the apical electrode position, and conversely with NBN or PSHC for the middle electrode position. These results provide evidence that the choice of acoustic stimulus type can have a significant effect on electric-acoustic pitch matching.
Purpose: To investigate short-term (3 months follow-up) changes in visual quality following Descemet membrane endothelial keratoplasty (DMEK) for Fuchs endothelial dystrophy (FED). Methods: In this prospective institutional case series, 51 patients that underwent DMEK for FED were included. Assessment included the Quality of Vision (QoV) questionnaire preoperatively, at 1 month, and 3 months after surgery. Secondary outcome measures were anterior segment parameters acquired by Scheimpflug imaging, corrected distance visual acuity (CDVA), and endothelial cell density (ECD). Results: Glare, hazy vision, blurred vision, and daily fluctuation in vision were the symptoms mostly reported preoperatively. All symptoms demonstrated a significant reduction of item scores for severity, frequency, and bothersome in the course after DMEK (P < 0.01). Glare and fluctuation in vision remained to some extent during the follow-up period (median score = 1). Preoperatively, corneal densitometry correlated moderately to weakly with severity of hazy vision (rs = 0.39; P = 0.03) and frequency (rs = 0.26; P = 0.02) as well as severity (rs = 0.27; P = 0.03) of blurry vision. CDVA and central corneal thickness (CCT) did not correlate with visual complains. Conclusions: Following DMEK for FED, patient-reported visual symptoms assessed by the QoV questionnaire represent a useful tool providing valuable information on the impact of DMEK on visual quality that cannot be directly estimated by morphological parameters and visual acuity only.
