Ziel der vorliegenden Arbeit war die Untersuchung der Kurzzeitdynamik zirkulierender Tumorzellen bei hepatozellulären Karzinompatienten, die erstmalig eine radiologische Intervention in Form einer transarteriellen Chemoembolisation oder einer Mikrowellenablation als Therapie erhielten. Dafür wurde in Vorversuchen eine neuartige Methode zur Isolation und Detektion von zirkulierenden Tumorzellen entwickelt.
Zugleich sollte ein potenzieller Einsatz dieser Methode als Screeningverfahren für hepatozelluläre Karzinompatienten mithilfe der Sensitivität und Spezifität überprüft werden. Darüber hinaus wurde eine mögliche Korrelation der Kurzzeitdynamik zirkulierender Tumorzellen sowohl mit der Kurzzeitdynamik des AFP-Wertes als anerkannten HCC-Tumormarker, als auch des IL-6-Wertes als aktuell diskutierten Tumormarker analysiert. Ferner wurde die Kurzzeitdynamik zirkulierender Tumorzellen mit dem klinischen Verlauf der Patienten verglichen.
Im Zeitraum von September 2017 bis Juni 2018 konnten Blutproben von 18 Patienten mit einem hepatozellulären Karzinom untersucht werden. Davon wurden zehn Patienten mittels Mikrowellenablation und acht Patienten mittels transarterieller Chemoembolisation behandelt. Daneben wurden Blutproben von 13 gesunden Probanden ausgewertet.
Methodisch wurden jeweils im Anschluss an die Gewinnung des Patientenblutes, vor und unmittelbar nach jeweiliger radiologischer Intervention, die zirkulierenden Tumorzellen aus dem Blut isoliert und die gewonnenen Zellen durch die Durchflusszytometrie nachgewiesen und quantifiziert. Als zirkulierende Tumorzellen wurden dabei jene Zellen betrachtet, die eine Negativität auf den Marker CD45 sowie eine Positivität auf die Marker ASGPR-1, CD146, CD274 und CD90 zeigten.
Durch den Vergleich der Proben vor und nach radiologischer Intervention konnte gezeigt werden, dass die Anzahlen zirkulierender Tumorzellen nach der radiologischen Intervention im Mittel niedriger sind als vor der Therapie. Die Anzahl zirkulierender Tumorzellen sank dabei im Schnitt bei den mit Mikrowellenablation behandelten Patienten stärker im Vergleich zu denen, die eine transarterielle Chemoembolisation erhielten. Dies bestätigte den klinischen Verlauf der Patientengruppen, da die Mortalität der Gruppe, die eine transarterielle Chemoembolisation erhielten deutlich höher war als bei den übrigen Patienten.
Als klinisch einsetzbare Screening-Methode im Sinne einer „Liquid Biopsy“ für das HCC müssen die Sensitivität und Spezifität weitergehend optimiert werden. Eine signifikante Korrelation zwischen der Kurzzeitdynamik der Anzahlen zirkulierender Tumorzellen und der Kurzzeitdynamik der AFP-Werte und der IL-6-Werte konnte nicht festgestellt werden. Entgegen des erwarteten Trends, legte die angewendete Methode, durch das Absinken der zirkulierenden Tumorzellen, die positive Wirkung der transarteriellen Chemoembolisation und der Mikrowellenablation in hepatozellulären Karzinompatienten dar.
Patienten mit akuter Dekompensation einer Leberzirrhose (AD) oder einem akut-auf-chronischen-Leberversagen (ACLF) stellen ein vulnerables Kollektiv für den Erhalt eines Erythrozytenkonzentrates (EK) dar. Zu den Ursachen zählen das häufige Auftreten einer gastrointestinalen Blutung, die Koagulopathie oder das Vorliegen einer chronischen Anämie. Während für viele andere Patientenkollektive das richtige Transfusionsmanagement bereits erforscht worden ist, fehlen diese Studien für Patienten mit Leberzirrhose, insbesondere für die neue Entität ACLF. Die vorliegende Studie soll die Auswirkung einer EK-Transfusion auf Morbidität und Mortalität dieser Patienten untersuchen.
Insgesamt wurden 498 Patienten mit der Diagnose einer Leberzirrhose, die zwischen den Jahren 2015 und 2019 auf eine Intensivstation der Universitätsklinik Frankfurt aufgenommen worden sind, retrospektiv analysiert. Für die statistische Auswertung wurde ein Prospensity-Score-Matching nach EK-Transfusion mit Adjustierung für mögliche Konfundierungseffekte durchgeführt. Der Einfluss der Transfusion auf die Mortalität wurde mithilfe von Kaplan-Meier-Kurven und multivariater Cox-Regression untersucht. Für die ACLF-Kohorte wurden ROC-Kurven zum Versuch der Identifizierung eines Transfusionstriggers und eines Zielhämoglobinwertes nach Transfusion angefertigt.
In der Gesamtkohorte wiesen transfundierte Patienten eine signifikant höhere Mortalitätsrate als Nicht-Transfundierte auf (28-Tages-Mortalität: 39,6% vs. 19,5%, p<0,001). Dabei wirkte sich die Transfusion primär bei Patienten mit ACLF negativ auf das Überleben aus. Nach Matching der Patienten nach Erhalt eines EKs und Adjustierung für potentielle Konfundierungseffekte blieb die Kurzzeitmortalität bei transfundierten Patienten mit ACLF weiterhin signifikant erhöht (28-Tages-Mortalität: 72,7% vs. 45,5%, p=0,03). Bei AD Patienten zeigte die Transfusion keinen signifikanten Einfluss auf das Überleben. Die multivariate Cox-Regression identifizierte die EK-Transfusion als unabhängigen Risikofaktor der Kurzzeitmortalität im ACLF (HR: 2,55; 95% KI 1,26 – 5,15, p=0,009). Die Identifizierung eines Transfusionstriggers oder eines Zielhämoglobinwertes war nicht möglich.
Die vorliegende Studie konnte zeigen, dass die EK-Transfusion einen Risikofaktor der Kurzzeitmortalität im ACLF, aber nicht für AD Patienten darstellt. Dieses Ergebnis macht fortführende Untersuchungen zur Ursachenforschung und zur Evaluierung des richtigen Transfusionsmanagements für Patienten mit ACLF notwendig.
Acne inversa ist eine chronisch entzündliche Hauterkrankung der Terminalhaarfollikel und Talgdrüsen, die sich zu schmerzhaften tiefsitzenden Knoten entwickelt, welche in Abszessen und Fistelgängen resultieren können und mit starken Schmerzen und psychischen Belastungen für die Patienten einhergehen. Die Pathophysiologie der AI ist bisher nur unzureichend verstanden. Es wird angenommen, dass die IL-23-TH17-IL-17-Achse eine wichtige Rolle in der Pathogenese der AI spielt. Neben der Hyperkeratose im Bereich des Terminalhaarfollikels scheinen die entzündlichen Infiltrate im Bereich der Epidermis eine psoriasiforme Hyperplasie zu induzieren. In vorangegangenen Arbeiten konnte gezeigt werden, dass der mTORC1-Signalweg (mammalian target of rapamycin complex 1), welcher durch Zytokine wie IL-1β, TNF-α und IL-17A aktiviert wird, in der Pathogenese der Psoriasis vulgaris von großer Bedeutung ist. Aufgrund immunologischer und histologischer Gemeinsamkeiten beider Erkrankungen ist es denkbar, dass der mTORC1-Signalweg ebenfalls bei der Pathogenese und Progression der AI eine Rolle spielt, was im Rahmen dieser Arbeit untersucht werden sollte. Immunhistochemische Färbungen für phosphorylierte Komponenten des Signalwegs zeigten eine stark erhöhte mTORC1-Aktivität in den AI-Läsionen. Diese war abhängig vom Schweregrad der AI-Läsion sogar teilweise höher als in der Psoriasis vulgaris. Die starke Aktivierung der mTORC1-Kaskade korrelierte mit Stellen, die eine aberrante Expression von Differenzierungs-, Proliferations- und Entzündungsmarkern aufwiesen. Auffällig war ebenfalls die starke STAT3-Aktivierung, welche durch erhöhte Phosphorylierung an Y705 und S727 gemessen werden konnte und auch auf eine Beteiligung dieses Signalwegs an der Pathogenese hindeutet. Da es Hinweise auf Überschneidungen zwischen dem mTORC1-Signalweg und der ebenfalls in der Psoriasispathogenese involvierten STAT3- Kaskade gibt, wurde dieser Zusammenhang untersucht. Es konnte in vitro gezeigt werden, dass psoriasis-typische Zytokine eine Phosphorylierung von STAT3 an S727 induzieren, was durch die Behandlung mit dem mTOR-Inhibitor Rapamycin gehemmt werden konnte.
Zusammenfassend deuten die hier gewonnenen Daten darauf hin, dass der PI3-K/Akt/mTOR-Signalweg, aber auch die JAK/STAT3-Kaskade eine entscheidende Rolle in der Acne inversa-Pathogenese spielen und damit potenziell neue Angriffspunkte für die Entwicklung neuer Therapien darstellen können. Damit geben die gezeigten Ergebnisse vielversprechende Ansatzpunkte um pharmakologisch gut etablierte Medikamente wie z.B. Sirolimus oder Tofacitinib als neue Ansätze für die AI-Therapie weiter zu untersuchen.
Hintergrund: Für die Langzeitbehandlung vieler chronischen Erkrankungen im Kindesalter ist ein dauerhafter zentralvenöser Zugang notwendig, der die Applikation von Medikamenten und eine parenterale Ernährung sichert. Häufig werden hierfür sogenannte tunnelierte zentralvenöse Katheter wie Hickman-/Broviac- Katheter verwendet. Diese Kathetersysteme weisen spezifische postoperative Komplikationen auf. Katheterassoziierte Komplikationen sind von großer klinischer Relevanz, da es hierdurch zu einer verfrühten Explanation des Katheters kommen kann und die Applikation von Medikamenten nicht mehr gewährleistet ist. Es ist daher entscheidend, die Risiken postoperativer Komplikationen zu identifizieren, um entsprechende Präventions- und Verhaltensregeln zu entwickeln und die Verweildauer des tunnelierten Katheters möglichst langfristig sicherzustellen.
Ziel der vorliegenden retrospektiven Studie war neben der Untersuchung postoperativer Komplikationen tunnelierter Hickman-/Broviac- Katheter im Kindesalter die Identifikation von prädisponierenden Risikofaktoren.
Material und Methoden: In einer retrospektiven Kohortenstudie wurden in dem Zeitraum von Januar 2013 bis Juni 2017 Daten von pädiatrischen Patient:innen bis 21 Jahre mit einliegenden Hickmann-/Broviac Kathethersystemen eingeschlossen. Mögliche Risikofaktoren bezüglich katheterassoziierter Komplikationen wurden analysiert: Alter, Geschlecht, Grunderkrankung, Behandlungsindikation, das zur Implantation verwendete Gefäß, Anzahl der Lumen und die Kathetertage in situ. Die Daten wurden aus Papierakten, digitalen Krankenakten, Operationsberichten und Röntgenaufnahmen der Klinik für Kinderheilkunde und der Klinik für Kinderchirurgie und Kinderurologie des Universitätsklinikums Frankfurt in Excel (Version 2016) anonymisiert erfasst und deskriptiv und explorativ ausgewertet. Signifikante Unterschiede wurden mit Hilfe des X2-Test, des exakten Fisher-Tests, sowie des Mann- Whitney-U-Tests ermittelt. Die Durchführung der Studie wurde vom Ethikkomitee des Universitätsklinikums Frankfurt am Main genehmigt (Nummer 172-18).
Ergebnisse: Insgesamt wurden 333 Patient:innen mit 386 implantierten Kathetern (BARD Hickman-/Broviac- zentralvenöser Katheter; Bard Access Systems, Salt Lake City, UT), in die Studie eingeschlossen. Der Altersmedian der Patient:innen zum Zeitpunkt der Implantation lag bei 6.34 Jahren. Die meisten Katheter wurden wegen einer malignen Erkrankungen implantiert, die entsprechend mit einer Chemotherapie, Stammzelltransplantation und/oder Radiotherapie behandelt wurden. Die Explantation der Katheter erfolgte in der Regel mit der Beendigung der Therapie. In 22% (85 von 386 tunnelierten Kathetern) traten postoperative Komplikationen auf, mit einer kumulativen Ein-Jahres-Inzidenz von 21 % und 0.93 Komplikationen pro 1000 Kathetertagen. Am häufigsten waren Katherinfektionen (0.56 pro 1000 Kathetertagen) gefolgt von Dislokationen (0.29 pro 1000 Kathetertagen). Folgende Risikofaktoren für postoperative Komplikationen wurden identifiziert: Ein Patient:innenenalter von unter einem Lebensjahr, eine parenterale Ernährung, eine Stammzelltransplantation bei nicht malignen Grunderkrankungen, eine hämatologische Erkrankung und Tumore des Zentralen Nervensystems, eine offen chirurgische Implantation des Katheters, die V. jugularis interna als Zielgefäß und die Anwendung einlumiger Katheter. Tunnelierte Katheter, die für die Behandlung von einer Leukämie, einem Lymphom, oder eines soliden Tumors implantiert wurden, wiesen am wenigsten Komplikationen auf.
Schlussfolgerung/Ausblick: Insgesamt zeigt die vorliegende Studie eine mit der internationalen Literatur vergleichbare hohe Rate an postoperativen Komplikationen auf. Katheterinfektionen und Katheterdislokationen sind dabei die häufigsten Komplikationen und sollten daher besondere Beachtung im klinischen Alltag finden. Ferner konnten spezifische, risikobehaftete Patient:innengruppen identifiziert werden. Insbesondere Patient:innen, welche eine parenterale Ernährung erhielten, wiesen Katheterinfektionen auf. Daraus ist abzuleiten, dass insbesondere diesen Patient:innen eine engmaschige Überwachung zu kommen sollte, um frühzeitig Komplikationen zu detektieren und diesen entgegenzuwirken.
Die Gesamtzahl der untersuchten Katheter ist groß, einzelne Subgruppen fallen aber klein aus. Sie beeinflussen sich teilweise gegenseitig. In Zukunft sollten multivariate Studien für eine genauere Extrahierung einzelner Risikofaktoren erfolgen.
Die vorliegende Arbeit thematisiert den Vergleich der Bildqualität von Liegend-Röntgen-Thorax-Aufnahmen von Patienten auf der Intensivstation des Universitätsklinikums Frankfurt unter Verwendung einerseits eines parallelen und andererseits eines virtuellen Streustrahlenrasters (=Bildverarbeitungssoftware). Es wurde untersucht, ob mit dem virtuellen Raster eine mindestens gleichwertige Bildqualität wie mit dem parallelen Raster erreicht und gleichzeitig Strahlendosis eingespart werden kann.
Insgesamt wurden 378 Röntgen-Thorax-Aufnahmen von 126 Patienten, die jeweils einmal mit parallelem Raster, mit virtuellem Raster und mit dem gleichen virtuellen Raster mit Dosisreduktion durchgeführt wurden, in die Studie eingeschlossen. Das virtuelle Raster ahmt das parallele Raster in der Streustrahlenreduktion nach. Das Übergewicht der Patienten als Einschlusskriterium der Studie rechtfertigte den Einsatz des parallelen Rasters. Jeder Patient wurde nur nach klinischer Indikationsstellung geröntgt, sodass der zeitliche Abstand zwischen zwei Röntgen-Thorax-Aufnahmen unterschiedlicher Aufnahmetechniken desselben Patienten variierte. Für alle Röntgen-Thorax-Aufnahmen wurde derselbe indirekte Flachdetektor verwendet. Die Röhrenspannung betrug konstant 125 kV, das Strom-Zeit-Produkt 1,4 mAs (für das parallele und virtuelle Raster) bzw. 1,0 mAs (für das virtuelle Raster mit Dosisreduktion). Für jeden Röntgen-Thorax wurde das Dosisflächenprodukt bestimmt. Vier Radiologen evaluierten die Bildqualität hinsichtlich sechs Kriterien (Lungenparenchym, Weichteile, thorakale Wirbelsäule, Fremdkörper, Pathologien und Gesamtqualität) anhand einer 9-Punkte-Skala. Der Friedman-Test (p < 0,05: signifikant) wurde angewendet. Die Übereinstimmung der Radiologen wurde über Intraklassenkorrelationskoeffizienten berechnet.
Das virtuelle Raster ohne/mit Dosisreduktion wurde insgesamt von allen vier Radiologen für die Weichteile, die thorakale Wirbelsäule, die Fremdkörper und die Gesamtbildqualität signifikant besser bewertet als das parallele Streustrahlenraster (p ≤ 0,018).
Für das Lungenparenchym und die Pathologien resultierten sowohl signifikante als auch nicht-signifikante Ergebnisse, wobei bei signifikanten Ergebnissen ebenfalls das virtuelle Raster ohne/mit Dosisreduktion besser bewertet wurde als das parallele Streustrahlenraster (p ≤ 0,002). Einzige Ausnahme stellten die Evaluationen der Bildqualität bez. des Lungenparenchyms eines Radiologen dar, der das virtuelle Raster ohne/mit Dosisreduktion signifikant schlechter bewertete als das parallele Raster (p < 0,0001). Insgesamt wurde das virtuelle Raster mit Dosisreduktion für die folgenden Kriterien am besten in absteigender Reihenfolge im Vergleich zum parallelen Raster bewertet: Fremdkörper, thorakale Wirbelsäule, Weichteile, Gesamtbildqualität, Pathologien und Lungenparenchym. Die Übereinstimmung der vier Radiologen in ihren Bildqualitätsbewertungen war maximal gering. Mit dem virtuellen Raster wurde im Durchschnitt etwa 28,7% des Dosisflächenprodukts im Vergleich zum parallelen Streustrahlenraster eingespart (p < 0,0001).
Bisher haben nur vier Studien Streustrahlenreduktionssoftwares an Liegend-Röntgen-Thorax-Aufnahmen untersucht, davon zwei an lebenden Menschen. Limitationen der vorliegenden Studie sind die Subjektivität der Bewertungen der Radiologen, die mögliche Identifizierung der Röntgen-Thorax-Aufnahmen, die mit dem parallelen Streustrahlenraster als gängige Aufnahmetechnik in der Radiologie des Universitätsklinikums Frankfurt durchgeführt wurden, die Konstanz der Expositionsparameter unabhängig des BMI der Patienten und die eingeschränkte Vergleichbarkeit der Röntgen-Thorax-Aufnahmen desselben Patienten aufgrund von Veränderungen der Pathologien, Fremdkörper, etc. bei (großem) zeitlichem Abstand zwischen den Röntgen-Thorax-Aufnahmen.
Das virtuelle Raster erzielte teils eine gleichwertige, teils eine bessere Bildqualität wie/als das parallele Raster bei gleichzeitiger Dosisreduktion von 28,7% und kann es somit bei Liegend-Röntgen-Thorax-Aufnahmen ersetzen. Weitere Studien sollten den Einsatz des virtuellen Rasters bei Röntgenaufnahmen des Thorax (stehend und liegend) und anderer Körperpartien im Hinblick auf die Bildqualität, (höhere) Dosiseinsparungen und den Workflow untersuchen.
Knochendefekte sind ein häufig anzutreffendes Krankheitsbild mit multiplen Ursachen wie zum Beispiel Trauma, Infektion und Tumore. Besondere Aufmerksamkeit bedürfen dabei Knochendefekte kritischer Größe (CSD). Diese sind definiert als ein Knochendefekt, der trotz adäquater Stabilisierung in der Lebzeit des Patienten nicht abheilen wird. Gerade diese sind in der Versorgung sehr zeit- und geldintensiv und stellen eine große Belastung für den Patienten dar. Trotz deren Häufigkeit finden sich nur Behandlungsverfahren, die mit signifikanten Nachteilen einhergehen. Daher ist gerade in der Behandlung von CSD weitere Forschung und die Entwicklung neuer Behandlungsmöglichkeiten nötig. Ein gut belegtes und klinisch erfolgreich angewandtes Verfahren stellt die Masquelet-Technik dar, welche ein zweizeitiges Verfahren ist, bei dem zunächst in einer ersten OP der Defekt ausgiebig durch Debridieren und die Zugabe von Antibiotika vorbereitet wird und anschließend mit einem Spacer aus Knochenzement und einem geeigneten Osteosyntheseverfahren stabilisiert wird. Über mehrere Wochen bildet sich nun eine Biomembran, die Masquelet-Membran, um den Spacer, welche in einer zweiten OP eröffnet wird. Daraufhin wird der Spacer entfernt und zum Beispiel mit autologem Knochen aus dem Beckenkamm gefüllt. Dann wird die Membran wieder verschlossen und der Defekt kann abheilen. Mit diesem Verfahren können auch große Defekte versorgt werden und in der Literatur finden sich Erfolge bei bis zu 25cm langen Defekten. Limitierungen finden sich hier in der Verfügbarkeit von autologem Knochenmaterial. Hier kann mit synthetischen Knochenersatzstoffen, wie zum Beispiel β-TCP, oder xenogenen Materialen zum Beispiel vom Rind gearbeitet werden. Einen weiteren großen Nachteil sahen wir jedoch in der Notwendigkeit einer zweiten OP. Dies setzt den Patienten erneut dem Operations- und Narkoserisiko aus, ist mit erneuten Schmerzen verbunden und führt zu verlängerten Krankenhausaufenthalten mit gesteigerten Kosten. Um dies zu umgehen, wurde in dieser Arbeit die Masquelet-Membran durch eine Membran aus dezellularisierter humaner Dermis (Epiflex) ersetzt, welche bereits erfolgreich bei der Behandlung größerer Bauchwanddefekte eingesetzt wird. So könnte man in einer einzigen OP den Defekt mit der Dermis umhüllen und direkt mit dem gewünschten Knochenersatzmaterial füllen. Hypothese war, dass eine Defektumhüllung mit Epiflex die Defektheilung vergleichbar zur induzierten Membran unterstützt. Eine weitere Fragestellung dieser Arbeit war, ob durch den Einsatz von mononukleären Zellen des Knochenmarks (BMC) die Knochenheilung zusätzlich verbessert werden kann. Zu diesem Zweck wurden sechs Gruppen aus männlichen Sprague-Dawley-Ratten gebildet, welche einen 1 cm starken Defekt im Femur gesetzt bekamen. Die Gruppen 1 und 2 wurden mit dem Masquelet-Verfahren, Gruppe 1 ohne und Gruppe 2 mit Zugabe von BMC, behandelt. Die Gruppen 3 bis 6 wurden mit dem einzeitigen Verfahren unter Verwendung der Dermis als Defektumhüllung und verschiedenen Kombinationen von BMC in einer einzigen OP behandelt. Nach 8 Wochen wurden die operierten Femora, inklusive jeweils eines nicht operierten Referenzfemur pro Gruppe explantiert und histologisch aufgearbeitet. Mittels der MOVAT Färbung wurde die Knochenneubildung im Defekt und die histologische Transformation der Membran in knöchernes Gewebe beurteilt. Es konnte ein signifikanter knöcherner Umbau der Epiflex-Membran beobachtet werden. Da mit der Epiflex-Membran ein Fremdkörper in den Organismus eingebracht wurde, wurde auch das Inflammationsgeschehen anhand histologischer Färbungen für Monozyten/Makrophagen (CD68) sowie für Granulozyten (CAE) beurteilt. Dabei zeigten sich keine signifikanten Unterschiede zwischen den Verfahren. Allerdings konnte eine unterschiedliche Verteilung der Immunzellen nachgewiesen werden. Abschließend wurde anhand von immunhistologischen α-SMA und vWF-Färbungen die Vaskularisierung im Defekt beurteilt. Die Epiflex-Membran wurde in gleichem Maße wie die induzierte Membran von Gefäßen durchdrungen. Dabei wuchsen häufig Gefäße aus dem umliegenden Muskelgewebe in den Defekt ein. Die zusätzliche Transplantation von BMC, welche eine zusätzliche Variable in dieser Arbeit darstellten, beeinflusste die Knochenheilung positiv. In Gruppen mit zusätzlichen BMC wurde geringere Inflammation und eine verbesserte Verknöcherung im Defektbereich beobachtet. Die Gefäßdichte wurde durch zusätzliche BMC jedoch nicht erhöht. Abschließend lässt sich aus den Ergebnissen schließen, dass der Ersatz der Masquelet-Membran durch die einzeitige Versorgung mit einer humanen dezellularisierten Dermis zu mindestens gleichwertigen Ergebnissen im Vergleich zur zweizeitigen induzierten Membrantechnik nach Masquelet führt.
Der Anspruch in der Implantatprothetik, ein hohes Maß an Qualität bei der Funktionalität und Ästhetik zu erreichen, hat zur Entwicklung verschiedener Implantatsysteme geführt. Deren Komponenten sollten nicht nur belastbar, sondern zugleich möglichst gewebeverträglich sein. Große Bedeutung kommt hier dem Bindeglied zwischen enossalem Implantat und der implantatgetragenen Krone, dem Abutment, zu. Dieses macht zumeist lediglich ein kleines Segment des transmukosalen Übergangs aus, zeigt sich jedoch für multiple Risikofaktoren, die den Langzeiterfolg der Implantatversorgung bedingen, verantwortlich.
Zirkonoxidabutments bestechen durch ihre Ästhetik und sehr gute Gewebeverträglichkeit. Titanabutments sind solchen aus Zirkonoxid mechanisch vor allem durch ihr charakteristisches Verhalten im Bereich der IAV überlegen. Um deren Vorteile zu vereinen, werden Standardabutments aus Titan mit einer individualisierten Zirkonoxidsuprakonstruktion adhäsiv zu einer funktionellen Einheit verbunden. Zur Verklebung dieser Hybridabutments eignen sich moderne Befestigungskomposite. Die Belastbarkeit dieser Verbindung kann durch verschiedene In-vitro-Untersuchungen bestätigt werden. Die Verbundfestigkeit kann durch Parameter wie den Zementspalt oder die Höhe des Titanabutments divergieren. Der Prozess der Verklebung muss einen hydrolysestabilen adhäsiven Verbund im feuchten Milieu der Mundhöhle gewährleisten, ohne die physikalischen Eigenschaften der eingesetzten Komponenten zu verändern.
Die Vermeidung bakterieller Kontamination der Hybridabutments erfolgt durch die Sterilisation im Autoklav. Da die Hybridabutments der semikritischen Risikogruppe der MP zugeordnet werden konnten, wurden diese häufig vor dem klinischen Einsatz jedoch nicht sterilisiert. Die Anwendung eines Sterilisationsverfahrens begünstigt sowohl den Zustand des periimplantären Weichgewebes, als auch die Stabilität der IAV bei Hybridabutments, geht allerdings mit einer thermischen Belastung einher. Verfahren zum Lösen von Klebeverbindungen nutzen die Eigenschaft von Klebstoffen, bei kritischem Wärmeeintrag mit einer Schädigung der Polymermatrix zu reagieren. Daher wurde die Hypothese formuliert, dass die Sterilisation im Autoklav den adhäsiven Verbund schwächen und zu niedrigeren Haftwerten führen könnte.
Zur Beurteilung eines möglichen Einflusses der Sterilisation auf die mechanische Widerstandsfähigkeit der verklebten Hybridabutments wurden mehrere Befestigungskomposite in die Untersuchung miteinbezogen. Es wurden 80 Titanbasen angefertigt und adhäsiv mit Zirkonoxidsuprakonstruktionen befestigt. Bei den fünf Befestigungskompositen handelte es sich um den DTK Kleber (Bredent), G-CEM LinkAce (GC Germany), RelyX Unicem2 (3M Espe AG), Multilink Hybridabutment (Ivoclar Vivadent GmbH) und Panavia F2.0 (Kuraray Medical). Von den fünf Gruppen mit je 16 Prüfkörpern durchlief jeweils die Hälfte eine Sterilisation im Autoklav während die Kontrollgruppe bei Raumtemperatur gelagert wurde. Abschließend wurden die Prüfkörper mit einer Universalprüfmaschine in uniaxialer Richtung bis zum Versagen der Verbindung belastet. Die dabei auftretende Abzugskraft wurde aufgezeichnet und die jeweiligen Werte miteinander verglichen.
Bei der Auswertung lagen statistisch signifikante Unterschiede der Haftwerte zwischen einer Test- und Kontrollgruppe, sowie unter den Befestigungskompositen selbst vor. Die Ergebnisse zeigen eine große Varianz, entkräften aber die formulierte Hypothese, dass eine abschließende Sterilisation im Autoklav zu einer Reduktion der Haftkraft innerhalb der Hybridabutments führen könnte. Die Ergebnisse können im Gegenteil dahingehend interpretiert werden, dass die Widerstandsfähigkeit durch die Sterilisation sogar verstärkt wird. Dies stellt vor dem Hintergrund, dass die Sterilisation neben der Beseitigung einer bakteriellen Kontamination auch Debris reduziert und somit die Passung der Komponenten erhöht, eindeutige Vorteile gegenüber weniger effektiven Aufbereitungsmethoden dar. Die positiven Effekte der Sterilisation auf den Knochenerhalt und das periimplantäre Gewebe überwiegen die potentiellen Nachteile nach derzeitigem Kenntnisstand bei weitem.
Da in der vorliegenden Versuchsreihe jedoch nur die Abzugskräfte in axialer Richtung verglichen wurden und keine künstliche Alterung simuliert wurde, bedarf es weiterer Untersuchungen um die mechanische Belastbarkeit in vivo zu verifizieren.
Die Behandlung eines hepatozellulären Karzinoms (HCC) mittels transarterieller Chemoembolisation (TACE) hat einen hohen Stellenwert in einem multimodalen Therapiekonzept. Diese komplikationsarme, interventionelle Therapie wird stets kontrolliert durchgeführt. Im Rahmen dieser Studie sollte die postinterventionelle Sonografie mit der regelhaft durchgeführten CT-Abdomen verglichen werden. Hierbei wurde die kontrastmittelverstärkte Sonografie (contrast enhanced ultrasound, CEUS) in Verbindung mit einer quantitativen Auswertung der Signalintensitäten des Kontrastmittels über die Zeit (time intensity curve analysis, TICA) verwendet.
Das primäre Ziel der Studie war der Vergleich der Kontrastmittelsonografie und TICA mit dem Grad der Lipiodol-Einlagerung im CT-Abdomen. Hierbei wurde jeweils die größte mittels TACE behandelte Läsion untersucht. Es wurde hierzu die Hypothese aufgestellt, dass die Bewertung der Restperfusion des Tumors in der CEUS mit der Lipiodol-Einlagerung - als Surrogatparameter der Restperfusion - korreliert. Als sekundäres Ziel wurde die Untersuchung auf Nicht-Unterlegenheit der Sonografie gegenüber der CT-Abdomen in Bezug auf mögliche unerwünschte Wirkungen der TACE definiert. So sollte insgesamt geklärt werden, ob eine Ablösung des CTs postinterventionell nach TACE realisierbar ist.
In vorliegender Arbeit wurden insgesamt 175 Interventionen bei 89 Patienten mit HCC und TACE-Behandlung prospektiv eingeschlossen. Am Tag nach TACE wurde jeweils eine Sonografie mit CEUS und TICA sowie eine CT-Abdomen durchgeführt. Im Anschluss wurde das perfundierte Areal der Läsion in der CEUS und die Lipiodol-Einlagerung in der CT-Abdomen verglichen und jeweils in vier Kategorien (nach mRECIST) eingeteilt. Bei der Auswertung waren die Untersucher für das jeweils andere Ergebnis verblindet und zusätzlich wurde eine TICA durchgeführt. Des Weiteren wurde die Detektion von potenziellen Nebenwirkungen der TACE in der Sonografie mit der CT-Abdomen verglichen.
In Bezug auf das primäre Prüfziel konnte mittels Cohen’s Kappa zur Übereinstimmung der Bewertungen in CEUS und CT-Abdomen (Interraterreliabilität) keine statistisch signifikante Vergleichbarkeit gezeigt werden (κ=0,028, p=0,288). Mittels linear bzw. quadratisch gewichtetem Cohen’s Kappa zeigte sich ebenso keine Korrelation zwischen den Bewertungen in CEUS und CT-Abdomen (κ=0,016 bzw. κ=0,012). Bei der Auswertung der quantitativen, mittels TICA ermittelten, Signalintensitäten des Kontrastmittels konnte eine schwache Korrelation mit der Lipiodol-Einlagerung im CT-Abdomen gefunden werden (p=0,032, r=0,180). Ein signifikanter Unterschied zwischen den TICA-Messwerten im Zentrum und der Peripherie des Tumors konnte gezeigt werden (p=0,013). Die Beurteilung von unerwünschten Wirkungen einer TACE ist in vorliegender Arbeit nur bedingt möglich, da in diesem Patientenkollektiv klinisch nur sehr wenige Nebenwirkungen auftraten. Der Nachweis von Korrelaten möglicher Komplikationen, wie einer Dreischichtung der Gallenblasenwand, gelang zwar zuverlässig sowohl in der B-Bild Sonografie als auch in der CT-Abdomen, die Vergleichbarkeit ist jedoch wie oben genannt eingeschränkt.
Es konnte mit ausreichender statistischer Power gezeigt werden, dass die CEUS und die CT-Abdomen in Bezug auf das primäre Prüfziel abweichende Ergebnisse liefern. Weitere Studien mit verändertem Design zur Evaluation der Vorhersagekraft für das Outcome bzgl. des Gesamtüberlebens der Patienten nach TACE sind somit erforderlich, um eine belastbare Bewertung vornehmen zu können. Die bisher veröffentlichte Literatur legt eine gute Aussagekraft der Sonografie nahe. Für die TICA konnte in vorliegender Arbeit in Bezug auf die Lipiodol-Einlagerung im CT-Abdomen keine relevante Korrelation gezeigt werden. Die TICA muss zudem technisch weiterentwickelt werden, um die Implementierung in den klinischen Alltag zu erleichtern. Bisher erscheint der Einsatz aufgrund einer zeitintensiven Auswertung der generierten Daten im klinischen Alltag wenig praktikabel. Eine Ablösung des CTs bezüglich der Detektion von Komplikationen nach TACE erscheint jedoch möglich und vor dem Hintergrund der Strahlenbelastung des Patienten und der Kosten sinnvoll.
Es kann abschließend festgehalten werden, dass die CEUS mit TICA postinterventionell nach TACE eine sichere, kosteneffektive und aussichtsreiche Untersuchungsmodalität darstellt. Mit ggf. weiteren Studien scheint somit die Ablösung des CTs im klinischen Alltag mittelfristig realisierbar.
Die Multiple Sklerose (MS) ist die häufigste nicht-traumatische, autoimmun-vermittelte Erkrankung des zentralen Nervensystems (ZNS), welche vor allem bei jüngeren Patienten mit Invalidisierung und anhaltenden neurologischen Defiziten einhergehen kann.
Im Rahmen eines optimalen Therapiekonzepts wurden deshalb immer neuere und potentere Medikamente eingeführt. Mit den Sphingosin-1-Phosphat-Rezeptor-1 (S1P1) -Agonisten Fingolimod und Siponimod sind seit mehreren Jahren Medikamente auf dem Markt deren Wirksamkeit bewiesen, jedoch die genauen Wirkprinzipien noch nicht vollends verstanden sind. Angenommen wurde bisher eine Lymphozytendepletion aufgrund einer Hemmung der Lymphozyteninfiltation ins ZNS über den ubiquitär exprimierten, G-protein gekoppelten S1P1-Rezeptor. Neben Wirksamkeiten im Bereich des Immunsystems spielt der S1P1-Rezeptor und sein natürliches Substrat, das S1P, in vielen essenziellen Bereichen eine entscheidende Rolle, unter anderem in der Ausbildung und Reifung des vaskulären Systems in der Embryogenese.
Die genaue Untersuchung des S1P1-Signalwegs in-vivo gestaltete sich deshalb erschwert, da S1P1-Knock-Out-Mäuse einen letalen Phänotyp ausbilden. Jedoch deuten immer mehr Untersuchungen auch auf eine direkte S1P1-Rezeptor-vermittelte Wirksamkeit von Fingolimod auf Zellen des ZNS hin, somit eine Wirkung über die bisher bekannte Lymphozytenaffektion hinaus. Eine genaue Darstellung der im ZNS-beteiligten Zellen und ihrer S1P1-Aktivität gelang bisher auf zellulärer Ebene nicht.
Mit dem in dieser Arbeit genutzten Mausmodell der genmodifizierten S1P1-Signaling-Maus sollte erstmals eine lokoregionale und zelluläre Untersuchung der am S1P1-Signalweg beteiligten Zellen im Rahmen von physiologischen und experimentellen autoimmunen Enzephalomyelitis (EAE)-Bedingungen im ZNS erfolgen. Hierbei entspricht die EAE weitgehend einem tierexperimentellen Korrelat der menschlichen MS. Bei Aktivierung eines S1P1-Rezeptors bei der S1P1-Signaling-Maus erfolgt durch eine gekoppelte Signalkaskade eine konsekutive Expression eines Histonproteins, welches an ein grün-fluoreszierendes Protein gekoppelt ist. Es resultiert eine grüne Fluoreszenz des Zellkerns der betroffenen Zelle. Bei der Kontroll-Maus findet sich keine Kopplung zwischen Rezeptor und im Zellkern befindlicher Proteine.
Hierbei konnte mit Hilfe von Immunhistochemie sowie der quantitativen Methode der Durchflusszytometrie ein S1P1-Signaling in peripheren Organen wie beispielweise der im Rahmen der MS bedeutsamen Milz nachgewiesen werden. Dadurch eröffnen sich Einblicke in Migrationsverhalten und Zusammensetzung der Lymphozyten-Subtypen und deren S1P1-Signaling im Rahmen von physiologischen Bedingungen und unter EAE-Bedingungen.
Die Darstellung des S1P1-Signalings im ZNS, als Hauptmanifestationsort der MS, gelang unter Zuhilfenahme der EAE mit dem genmodifizierten Mausmodell jedoch nicht. Da sich keine Unterschiede in der GFP-Expression zwischen der Signaling-Maus und der heterozygoten Kontroll-Maus zeigen, sind keinerlei Rückschlüsse auf ein echtes S1P1-Signaling möglich. Es zeigen sich zwar deutliche Expressionsunterschiede des GFP im Vergleich erkrankter und gesunder Versuchstiere, Rückschlüsse auf eine echte S1P1-Aktivität konnten jedoch nicht getroffen werden.
Zusammenfassend eignet sich das hier genutzte Mausmodell der genmodifizierten S1P1-Maus zur Untersuchung peripherer Organe und ihrem S1P1-Signaling, z.B. zur Untersuchung kardiovaskulärer Fragestellungen oder zur dezidierteren Veranschaulichung peripher lymphatischer Prozesse.
Zur Untersuchung ZNS-eigener Zellen sowie zur Beantwortung der Frage, ob und wie sie über den S1P1-Rezeptor agieren, bedarf es jedoch noch der Entwicklung eines geeigneteren Tiermodells.
Die bereits erprobte Möglichkeit der Biolumineszenz zeigte in vorherigen Untersuchungen zwar eine S1P1-Aktivität in-vivo, jedoch sind hier keinerlei Untersuchungen auf zellulärer Basis möglich, sodass mit dem aktuellen Stand der Forschung ein direkter Nachweis der S1P1-Aktivität auf zellulärer Ebene im ZNS nicht möglich ist.
Gait analysis as a clinical examination method has been increasingly used in recent years. In particular, the external knee adduction moment was often used as a surrogate measure for internal medial knee joint loading, e.g., in elderly individuals with medial knee osteoarthritis. Therefore, the knee adduction moment is also associated with the progression of knee osteoarthritis. Children and adolescents with valgus malalignment have been found to experience a reduced external knee adduction moment, but internal knee joint contact forces, particularly in the lateral compartment, were not previously studied.
First, medial and lateral knee joint contact forces were studied using muskulosceletal modeling in young individuals with and without valgus malalignment treated by guided growth. In addition, a systematic literature review was conducted to explore the relationship between external joint moments and internal joint contact forces. Finally, this relationship was investigated in children and adolescents with and without valgus malalignment. Furthermore, we examined whether statistical models could be determined to accurately predict internal knee joint contact forces by commonly used parameters from three-dimensional gait analysis, such as external knee joint moments.
It was found that guided growth normalized knee joint contact forces after treatment. In addition, the static radiographic mechanical axis angle correlated better after the treatment when the patients showed a typical limb alignment compared to the correlation before guided growth with the valgus malalignment due to compensating strategies during gait. Furthermore, the systematic review showed that the peak medial knee joint contact force was best predicted by the knee adduction moment and even better together with the knee flexion moment in the first half of stance. However, for the second half of stance of the medial knee joint contact force and the entire stance of the lateral knee joint contact force, only low correlations with knee adduction and/or flexion moment were found. Finally, statistical models could be determined with high accuracy for both medial and lateral knee joint contact force, for both peaks in the first and second half of stance, and for both study groups of children and adolescents with and without valgus malalignment by including knee adduction and flexion moment as predictors.
These results demonstrate the importance of examining not only the external knee adduction moment but also the knee flexion moment and, even better, the medial and lateral knee joint contact forces when evaluating knee joint loading. With these statistical models, clinicians can predict the medial and lateral knee joint contact forces without the need to perform musculoskeletal simulations and can therefore use standard three-dimensional gait analysis parameters such as knee adduction and flexion moment. This can improve guided growth treatment in children and adolescents with valgus malalignment with regard to implantation or explantation of the growth restricting plates or to rebound. Instrumented gait analysis could be particularly helpful in borderline cases, as kinematic compensation mechanisms during gait may play a role and the static radiograph alone does not provide information about dynamic joint loads.
Erythropoietin (Epo) is a crucial hormone regulating red blood cell number and consequently the hematocrit. Epo is mainly produced in the kidney by interstitial fibroblast-like cells. Previously, we have shown that in cultures of the immortalized mouse renal fibroblast-like cell line FAIK F3-5, sphingosine 1-phosphate (S1P), by activating S1P1 and S1P3 receptors, can stabilize hypoxia-inducible factor (HIF)-2α and upregulate Epo mRNA and protein synthesis. In this study, we have addressed the role of intracellular iS1P derived from sphingosine kinases (Sphk) 1 and 2 on Epo synthesis in F3-5 cells and in mouse primary cultures of renal fibroblasts. We show that stable knockdown of Sphk2 in F3-5 cells increases HIF-2α protein and Epo mRNA and protein levels, while Sphk1 knockdown leads to a reduction of hypoxia-stimulated HIF-2α and Epo protein. A similar effect was obtained using primary cultures of renal fibroblasts isolated from wildtype mice, Sphk1−/−, or Sphk2−/− mice. Furthermore, selective Sphk2 inhibitors mimicked the effect of genetic Sphk2 depletion and also upregulated HIF-2α and Epo protein levels. The combined blockade of Sphk1 and Sphk2, using Sphk2−/− renal fibroblasts treated with the Sphk1 inhibitor PF543, resulted in reduced HIF-2α and Epo compared to the untreated Sphk2−/− cells. Exogenous sphingosine (Sph) enhanced HIF-2α and Epo, and this was abolished by the combined treatment with the selective S1P1 and S1P3 antagonists NIBR-0213 and TY52156, suggesting that Sph was taken up by cells and converted to iS1P and exported to then act in an autocrine manner through S1P1 and S1P3. The upregulation of HIF-2α and Epo synthesis by Sphk2 knockdown was confirmed in the human hepatoma cell line Hep3B, which is well-established to upregulate Epo production under hypoxia. In summary, these data show that sphingolipids have diverse effects on Epo synthesis. While accumulation of intracellular Sph reduces Epo synthesis, iS1P will be exported to act through S1P1+3 to enhance Epo synthesis. Furthermore, these data suggest that selective inhibition of Sphk2 is an attractive new option to enhance Epo synthesis and thereby to reduce anemia development in chronic kidney disease.
Abstract
Rheumatoid arthritis (RA) is associated with systemic osteoporosis, which leads to severe disability and low quality of life. Current therapies target osteoclasts to reduce bone degradation, but more treatment options would be required to promote bone protection by acting directly on osteoblasts (OB). Recently, the local production of dopamine in inflamed joints of RA has been observed. Thus, in this project, we aimed to determine the implication of the neurotransmitter dopamine in the bone formation process in RA. Dopamine receptors (DR) in the human bone tissue of RA or osteoarthritis (OA) patients were examined by immunohistochemistry. DR in isolated human osteoblasts (OB) was analyzed by flow cytometry, and dopamine content was evaluated by ELISA. Osteoclasts (OC) were differentiated from the PBMCs of healthy controls (HC) and RA patients. Isolated cells were treated with specific dopamine agonists. The effect of dopamine on mineralization was evaluated by Alizarin red staining. Cytokine release in supernatants was measured by ELISA. Osteoclastogenesis was evaluated with TRAP staining. OC markers were analyzed via real-time PCR and bone resorption via staining of resorption pits with toluidine blue. All DR were observed in bone tissue, especially in the bone remodeling area. Isolated OB maintained DR expression, which allowed their study in vitro. Isolated OB expressed tyrosine hydroxylase, the rate-limiting enzyme for dopamine production, and contained dopamine. The activation of D2-like DR significantly increased bone mineralization in RA osteoblasts and increased osteoclastogenesis but did not alter the expression of OC markers nor bone resorption. DR were found in the bone remodeling area of human bone tissue and dopamine can be produced by osteoblasts themselves, thus suggesting a local autocrine/paracrine pathway of dopamine in the bone. D2-like DRs are responsible for bone mineralization in osteoblasts from RA patients without an increase in bone resorption, thus suggesting the D2-like DR pathway as a possible future therapeutic target to counteract bone resorption in arthritis
Purpose: (i) To examine the criterion taller-than-wide (TTW) for the sonographic assessment of thyroid nodules in areas of iodine deficiency in terms of frequency, anatomical distribution within the thyroid gland and risk of malignancy. (ii) To develop a model for nodule growth in the thyroid gland. Methods: German multicenter study consisting of two parts. In the prospective part, thyroid nodules were sonographically measured in all three dimensions, location within the thyroid gland and contact to a protrusion-like formation (horn) in the dorsal position of thyroid gland was noted. In addition, further sonographic features such as the composition, echogenity, margins and calcifications were investigated. All nodules from the prospective part were assessed for malignancy as part of clinical routine at the decision of the treating physician adhering to institutionally based algorithms. In the retrospective part, only nodules with fine needle aspiration and/or histology were included. The risk of malignancy in TTW nodules was determined by correlating them with cyotological and histological results. Results: Prospective part: out of 441 consecutively evaluated thyroid nodules, 6 were found to be malignant (1.4%, 95% CI 0.6–2.7%). Among the 74 TTW nodules (17%), 1 was malignant (1%, 95% CI 0–4%). TTW nodules were more often located in the dorsal half of the thyroid than non-TTW nodules (factor 2.3, p = 0.01, 95% CI 2.1–2.5) and more often located in close proximity to a horn than non-TTW nodules (factor 3.0, p = 0.01, 95% CI 2.4–3.8). Retrospective part: out of 1315 histologically and/or cytologically confirmed thyroid nodules, 163 TTW nodules were retrieved and retrospectively analyzed. A TTW nodule was 1.7 times more often benign when it was dorsal (95% CI 1.1–2.5) and 2.5 times more often benign when it was associated with a horn (95% CI 1.2–5.3). The overall probability of malignancy for TTW nodules was 38% (95% CI 30–46%) in this highly preselected patient group. Conclusion: TTW nodules are common in iodine deficient areas. They are often located in the dorsal half of the thyroid gland and are frequently associated with a dorsal protrusion-like formation (horn) of the thyroid. Obviously, the shape of benign nodules follows distinct anatomical preconditions within the thyroid gland. The frequency of TTW nodules and their predominant benignity can be explained by a pole concept of goiter growth. The difference between the low malignancy risk of TTW nodules found on a prospective basis and the high risk found retrospectively may be the result of a positive preselection in the latter.
Purpose: As the population ages, the incidence of rectal cancer among elderly patients is rising. Due to the risk of perioperative morbidity and mortality, alternative nonoperative treatment options have been explored in elderly and frail patients who are clinically inoperable or refuse surgery.
Methods: Here we present technical considerations and first clinical experience after treating a cohort of six rectal cancer patients (T1‑3, N0‑1, M0; UICC stage I-IIIB) with definitive external-beam radiation therapy (EBRT) followed by image-guided, endorectal high-dose-rate brachytherapy (HDR-BT). Patients were treated with 10–13 × 3 Gy EBRT followed by HDR-BT delivering 12–18 Gy in two or three fractions. Tumor response was evaluated using endoscopy and magnetic resonance imaging of the pelvis.
Results: Median age was 84 years. All patients completed EBRT and HDR-BT without any high-grade toxicity (> grade 2). One patient experienced rectal bleeding (grade 2) after 10 weeks. Four patients (67%) demonstrated clinical complete response (cCR) or near cCR, there was one partial response, and one residual tumor and hepatic metastasis 8 weeks after HDR-BT. The median follow-up time for all six patients is 42 weeks (range 8–60 weeks). Sustained cCR without evidence of local regrowth has been achieved in all four patients with initial (n)cCR to date.
Conclusion: Primary EBRT combined with HDR-BT is feasible and well tolerated with promising response rates in elderly and frail rectal cancer patients. The concept could be an integral part of a highly individualized and selective nonoperative treatment offered to patients who are not suitable for or refuse surgery.
The analysis of postmortem protein degradation has become of large interest for the estimation of the postmortem interval (PMI). Although several techniques have been published in recent years, protein degradation-based techniques still largely did not exceed basic research stages. Reasons include impractical and complex sampling procedures, as well as highly variable protocols in the literature, making it difficult to compare results. Following a three-step procedure, this study aimed to establish an easily replicable standardized procedure for sampling and processing, and further investigated the reliability and limitations for routine application. Initially, sampling and processing were optimized using a rat animal model. In a second step, the possible influences of sample handling and storage on postmortem protein degradation dynamics were assessed on a specifically developed human extracorporeal degradation model. Finally, the practical application was simulated by the collection of tissue in three European forensic institutes and an international transfer to our forensic laboratory, where the samples were processed and analyzed according to the established protocol.
Mesenchymal stromal/stem cells and their derivates are the most promising cell source for cell therapies in regenerative medicine. The application of extracellular vesicles (EVs) as cell-free therapeuticals requires particles with a maximum regenerative capability to enhance tissue and organ regeneration. The cargo of mRNA and microRNA (miR) in EVs after hypoxic preconditioning has not been extensively investigated. Therefore, the aim of our study was the characterization of mRNA and the miR loading of EVs. We further investigated the effects of the isolated EVs on renal tubular epithelial cells in vitro. We found 3131 transcripts to be significantly regulated upon hypoxia. Only 15 of these were downregulated, but 3116 were up-regulated. In addition, we found 190 small RNAs, 169 of these were miRs and 21 were piwi-interacting RNAs (piR). However, only 18 of the small RNAs were significantly altered, seven were miRs and 11 were piRs. Interestingly, all seven miRs were down-regulated after hypoxic pretreatment, whereas all 11 piRs were up-regulated. Gene ontology term enrichment and miR-target enrichment analysis of the mRNAs and miR were also performed in order to study the biological background. Finally, the therapeutic effect of EVs on human renal tubular epithelial cells was shown by the increased expression of three anti-inflammatory molecules after incubation with EVs from hypoxic pretreatment. In summary, our study demonstrates the altered mRNA and miR load in EVs after hypoxic preconditioning, and their anti-inflammatory effect on epithelial cells.
Background: The factors driving the late phase of COVID-19 are still poorly understood. However, autoimmunity is an evolving theme in COVID-19’s pathogenesis. Additionally, deregulation of human retroelements (RE) is found in many viral infections, and has also been reported in COVID-19.
Results: Unexpectedly, coronaviruses (CoV) – including SARS-CoV-2 – harbour many RE-identical sequences (up to 35 base pairs), and some of these sequences are part of SARS-CoV-2 epitopes associated to COVID-19 severity. Furthermore, RE are expressed in healthy controls and human cells and become deregulated after SARS-CoV-2 infection, showing mainly changes in long interspersed nuclear element (LINE1) expression, but also in endogenous retroviruses.
Conclusion: CoV and human RE share coding sequences, which are targeted by antibodies in COVID-19 and thus could induce an autoimmune loop by molecular mimicry.
Background: Safety, tolerability and efficacy of granulocyte colony-stimulating factor (G-CSF) for mobilization of hematopoietic stem and progenitor cells (HSPCs) from healthy donors have been conclusively demonstrated. This explicitly includes, albeit for smaller cohorts and shorter observation periods, biosimilar G-CSFs. HSPC donation is non-remunerated, its sole reward being “warm glow”, hence harm to donors must be avoided with maximal certitude. To ascertain, therefore, long-term physical and mental health effects of HSPC donation, a cohort of G-CSF mobilized donors was followed longitudinally.
Methods: We enrolled 245 healthy volunteers in this bi-centric long-term surveillance study. 244 healthy volunteers began mobilization with twice-daily Sandoz biosimilar filgrastim and 242 underwent apheresis after G-CSF mobilization. Physical and mental health were followed up over a period of 5-years using the validated SF-12 health questionnaire.
Results: Baseline physical and mental health of HSPC donors was markedly better than in a healthy reference population matched for ethnicity, sex and age. Physical, but not mental health was sharply diminished at the time of apheresis, likely due to side effects of biosimilar G-CSF, however had returned to pre-apheresis values by the next follow-up appointment after 6 months. Physical and mental health slightly deteriorated over time with kinetics reflecting the known effects of aging. Hence, superior physical and mental health compared to the general healthy non-donor population was maintained over time.
Conclusions: HSPC donors are of better overall physical and mental health than the average healthy non-donor. Superior well-being is maintained over time, supporting the favorable risk–benefit assessment of volunteer HSPC donation.

A webinar series that was organised by the Academy of Pharmaceutical Sciences Biopharmaceutics focus group in 2021 focused on the challenges of developing clinically relevant dissolution specifications (CRDSs) for oral drug products. Industrial scientists, together with regulatory and academic scientists, came together through a series of six webinars, to discuss progress in the field, emerging trends, and areas for continued collaboration and harmonisation. Each webinar also hosted a Q&A session where participants could discuss the shared topic and information. Although it was clear from the presentations and Q&A sessions that we continue to make progress in the field of CRDSs and the utility/success of PBBM, there is also a need to continue the momentum and dialogue between the industry and regulators. Five key areas were identified which require further discussion and harmonisation.
Omicron is the evolutionarily most distinct severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) variant of concern (VOC) to date. We report that Omicron BA.1 breakthrough infection in BNT162b2-vaccinated individuals resulted in strong neutralizing activity against Omicron BA.1, BA.2, and previous SARS-CoV-2 VOCs but not against the Omicron sublineages BA.4 and BA.5. BA.1 breakthrough infection induced a robust recall response, primarily expanding memory B (BMEM) cells against epitopes shared broadly among variants, rather than inducing BA.1-specific B cells. The vaccination-imprinted BMEM cell pool had sufficient plasticity to be remodeled by heterologous SARS-CoV-2 spike glycoprotein exposure. Whereas selective amplification of BMEM cells recognizing shared epitopes allows for effective neutralization of most variants that evade previously established immunity, susceptibility to escape by variants that acquire alterations at hitherto conserved sites may be heightened.
Die traditionellen Behandlungspositionen der Zahnärzt/innen hinter, neben und vor dem/r Patienten/in führen zur asymmetrischen Neigung und Verdrehung des Kopfes sowie des Rumpfes. Die Folge können Fehlhaltungen sein, die Muskel-Skelett-Erkrankungen verursachen. Das erklärt wahrscheinlich die hohe Prävalenz bei Zahnärzt/innen und zahnmedizinischen Fachangestellten. Daher werden in dieser Übersicht mögliche Ursachen und Konsequenzen der Prävalenz sowie ergonomische Maßnahmen für diese Berufsgruppen aufgeführt. Zudem erläutern wir ergonomische Empfehlungen für die Sitzhaltung von Zahnärzt/innen auf Basis der vorhandenen Literatur.
Dieser Beitrag beschäftigt sich mit Arbeitsabläufen und physischen Risikofaktoren von Zahnärzt/innen (ZA) und Zahnmedizinischen Fachangestellten (ZFA), die zu gesundheitlichen Schäden des Muskel-Skelett-Systems führen. Dabei soll besonders auf das Arbeitsfeld „Patientenmund“ sowie die Arbeitsbelastung und deren Auswirkung auf die Gesundheit eingegangen werden. Ferner werden die optimale Sitzhaltung und physische Anforderungen statischer und repetitiver Behandlungspositionen sowie -haltungen von ZA und ZFA diskutiert.
Around 20% of the American population have chronic pain and estimates in other Western countries report similar numbers. This represents a major challenge for global health care systems. Additional problems for the treatment of chronic and persistent pain are the comparably low efficacy of existing therapies, the failure to translate effects observed in preclinical pain models to human patients and related setbacks in clinical trials from previous attempts to develop novel analgesics. Drug repurposing offers an alternative approach to identify novel analgesics as it can bypass various steps of classical drug development. In recent years, several approved drugs were attributed analgesic properties. Here, we review available data and discuss recent findings suggesting that the approved drugs minocycline, fingolimod, pioglitazone, nilotinib, telmisartan, and others, which were originally developed for the treatment of different pathologies, can have analgesic, antihyperalgesic, or neuroprotective effects in preclinical and clinical models of inflammatory or neuropathic pain. For our analysis, we subdivide the drugs into substances that can target neuroinflammation or substances that can act on peripheral sensory neurons, and highlight the proposed mechanisms. Finally, we discuss the merits and challenges of drug repurposing for the development of novel analgesics.
Background: Bacterial burden as well as duration of bacteremia influence the outcome of patients with bloodstream infections. Promptly decreasing bacterial load in the blood by using extracorporeal devices in addition to anti-infective therapy has recently been explored. Preclinical studies with the Seraph® 100 Microbind® Affinity Blood Filter (Seraph® 100), which consists of heparin that is covalently bound to polymer beads, have demonstrated an effective binding of bacteria and viruses. Pathogens adhere to the heparin coated polymer beads in the adsorber as they would normally do to heparan sulfate on cell surfaces. Using this biomimetic principle, the Seraph® 100 could help to decrease bacterial burden in vivo.
Methods: This first in human, prospective, multicenter, non-randomized interventional study included patients with blood culture positive bloodstream infection and the need for kidney replacement therapy as an adjunctive treatment for bloodstream infections. We performed a single four-hour hemoperfusion treatment with the Seraph® 100 in conjunction with a dialysis procedure. Post procedure follow up was 14 days.
Results: Fifteen hemodialysis patients (3F/12 M, age 74.0 [68.0–78.5] years, dialysis vintage 28.0 [11.0–45.0] months) were enrolled. Seraph® 100 treatment started 66.4 [45.7–80.6] hours after the initial positive blood culture was drawn. During the treatment with the Seraph® 100 with a median blood flow of 285 [225–300] ml/min no device or treatment related adverse events were reported. Blood pressure and heart rate remained stable while peripheral oxygen saturation improved during the treatment from 98.0 [92.5–98.0] to 99.0 [98.0–99.5] %; p = 0.0184. Four patients still had positive blood culture at the start of Seraph® 100 treatment. In one patient blood cultures turned negative during treatment. The time to positivity (TTP) was increased between inflow and outflow blood cultures by 36 [− 7.2 to 96.3] minutes. However, overall TTP increase was not statistical significant.
Conclusions: Seraph® 100 treatment was well tolerated. Adding Seraph® 100 to antibiotics early in the course of bacteremia might result in a faster resolution of bloodstream infections, which has to be evaluated in further studies.
Whereas the clinical approach in pediatric cancer patients with febrile neutropenia is well established, data on non-neutropenic infectious episodes are limited. We therefore prospectively collected over a period of 4 years of data on all infectious complications in children treated for acute lymphoblastic or myeloid leukemia (ALL or AML) and non-Hodgkin lymphoma (NHL) at two major pediatric cancer centers. Infections were categorized as fever of unknown origin (FUO), and microbiologically or clinically documented infections. A total of 210 patients (median age 6 years; 142 ALL, 23 AML, 38 NHL, 7 leukemia relapse) experienced a total of 776 infectious episodes (571 during neutropenia, 205 without neutropenia). The distribution of FUO, microbiologically and clinically documented infections, did not significantly differ between neutropenic and non-neutropenic episodes. In contrast to neutropenic patients, corticosteroids did not have an impact on the infectious risk in non-neutropenic children. All but one bloodstream infection in non-neutropenic patients were due to Gram-positive pathogens. Three patients died in the context of non-neutropenic infectious episodes (mortality 1.4%). Our results well help to inform clinical practice guidelines in pediatric non-neutropenic cancer patients presenting with fever, in their attempt to safely restrict broad-spectrum antibiotics and improve the quality of life by decreasing hospitalization.
Background: To minimize the risk of disease transmission in cornea transplantation, donor screening for blood-derived viral infections is mandatory. Ideally, pre-mortem blood samples are used, but based on availability, cadaveric blood samples of cornea donors may also be used. However, serological and nucleic acid amplification tests (NATs) need to be validated for the use of cadaveric specimens.
Methods: Hepatitis B virus (HBV), hepatitis C virus (HCV), human immunodeficiency virus (HIV), human T-lymphotropic virus (HTLV) 1/2, and Treponema pallidum (syphilis)-specific serological and/or NAT assays were validated on different platforms (Abbott Alinity i, Alinity m, Roche Cobas 6800, and Roche Cobas AmpliPrep/Cobas TaqMan (CAP/CTM)) using (un)spiked paired pre- and post-mortem cornea donor blood samples from the same individual (up to 23.83 h after death) of 28 individuals in accordance with the specifications of the German Federal Institute for Vaccines and Biomedicines (Paul-Ehrlich-Institut [PEI]). In addition, routinely HBV-, HCV- and HIV-PCR-negative tested post-mortem blood samples of 24 individuals were used to assess NAT specificity.
Results: For the majority of serological parameters on the Abbott Alinity i (HBsAg, anti-HBc, anti-HBs, anti-HCV, anti-HIV, anti-HTLV 1/2, and anti-Treponema pallidum), ratios of generated test results of (un)spiked paired pre- and post-mortem blood samples differed ≤25%, with an agreement of qualitative pre- and post-mortem test results ranging from 91.2 to 100%. For NAT parameters (HBV, HCV, and HIV) on the Cobas 6800, Alinity m, and CAP/CTM, no significant deviation in virus concentrations (factor >5) of spiked pre- and post-mortem blood samples could be observed. Ct-values of corresponding internal controls did also not differ significantly (>1.5 Ct-values). In addition, no false-positive test results were generated when specificity was assessed.
Conclusion: Overall, fluctuations of test results for serological and NAT parameters in pre- and post-mortem blood samples examined in this study, were only limited and within the range of what is also observed when routinely testing fresh patient specimens. We conclude that all examined assays are eligible for the screening of blood samples taken up to about 24 h after the occurrence of death.
Background and purpose: Impaired kidney function is associated with an increased risk of vascular events in acute stroke patients, when assessed by single measurements of estimated glomerular filtration rate (eGFR). It is unknown whether repeated measurements provide additional information for risk prediction.
Methods: The MonDAFIS (Systematic Monitoring for Detection of Atrial Fibrillation in Patients with Acute Ischemic Stroke) study randomly assigned 3465 acute ischemic stroke patients to either standard procedures or an additive Holter electrocardiogram. Baseline eGFR (CKD-EPI formula) were dichotomized into values of < versus ≥60 ml/min/1.73 m2. eGFR dynamics were classified based on two in-hospital values as “stable normal” (≥60 ml/min/1.73 m2), “increasing” (by at least 15% from baseline, second value ≥ 60 ml/min/1.73 m2), “decreasing” (by at least 15% from baseline of ≥60 ml/min/1.73 m2), and “stable decreased” (<60 ml/min/1.73 m2). The composite endpoint (stroke, major bleeding, myocardial infarction, all-cause death) was assessed after 24 months. We estimated hazard ratios in confounder-adjusted models.
Results: Estimated glomerular filtration rate at baseline was available in 2947 and a second value in 1623 patients. After adjusting for age, stroke severity, cardiovascular risk factors, and randomization, eGFR < 60 ml/min/1.73 m2 at baseline (hazard ratio [HR] = 2.2, 95% confidence interval [CI] = 1.40–3.54) as well as decreasing (HR = 1.79, 95% CI = 1.07–2.99) and stable decreased eGFR (HR = 1.64, 95% CI = 1.20–2.24) were independently associated with the composite endpoint. In addition, eGFR < 60 ml/min/1.732 at baseline (HR = 3.02, 95% CI = 1.51–6.10) and decreasing eGFR were associated with all-cause death (HR = 3.12, 95% CI = 1.63–5.98).
Conclusions: In addition to patients with low eGFR levels at baseline, also those with decreasing eGFR have increased risk for vascular events and death; hence, repeated estimates of eGFR might add relevant information to risk prediction.
Nodular lymphocyte-predominant Hodgkin lymphoma (NLPHL) can show variable histological growth patterns and present remarkable overlap with T-cell/histiocyte-rich large B-cell lymphoma (THRLBCL). Previous studies suggest that NLPHL histological variants represent progression forms of NLPHL and THRLBCL transformation in aggressive disease. Since molecular studies of both lymphomas are limited due to the low number of tumor cells, the present study aimed to learn if a better understanding of these lymphomas is possible via detailed measurements of nuclear and cell size features in 2D and 3D sections. Whereas no significant differences were visible in 2D analyses, a slightly increased nuclear volume and a significantly enlarged cell size were noted in 3D measurements of the tumor cells of THRLBCL in comparison to typical NLPHL cases. Interestingly, not only was the size of the tumor cells increased in THRLBCL but also the nuclear volume of concomitant T cells in the reactive infiltrate when compared with typical NLPHL. Particularly CD8+ T cells had frequent contacts to tumor cells of THRLBCL. However, the nuclear volume of B cells was comparable in all cases. These results clearly demonstrate that 3D tissue analyses are superior to conventional 2D analyses of histological sections. Furthermore, the results point to a strong activation of T cells in THRLBCL, representing a cytotoxic response against the tumor cells with unclear effectiveness, resulting in enhanced swelling of the tumor cell bodies and limiting proliferative potential. Further molecular studies combining 3D tissue analyses and molecular data will help to gain profound insight into these ill-defined cellular processes.
Purpose: Preoperative (neoadjuvant) chemoradiotherapy (CRT) and total mesorectal excision is the standard treatment for rectal cancer patients (UICC stage II/III). Up to one-third of patients treated with CRT achieve a pathological complete response (pCR). These patients could be spared from surgery and its associated morbidity and mortality, and assigned to a “watch and wait” strategy. However, reliably identifying pCR based on clinical or imaging parameters remains challenging.
Experimental design: We generated gene-expression profiles of 175 patients with locally advanced rectal cancer enrolled in the CAO/ARO/AIO-94 and -04 trials. One hundred and sixty-one samples were used for building, training and validating a predictor of pCR using a machine learning algorithm. The performance of the classifier was validated in three independent cohorts, comprising 76 patients from (i) the CAO/ARO/AIO-94 and -04 trials (n = 14), (ii) a publicly available dataset (n = 38) and (iii) in 24 prospectively collected samples from the TransValid A trial.
Results: A 21-transcript signature yielded the best classification of pCR in 161 patients (Sensitivity: 0.31; AUC: 0.81), when not allowing misclassification of non-complete-responders (False-positive rate = 0). The classifier remained robust when applied to three independent datasets (n = 76).
Conclusion: The classifier can identify >1/3 of rectal cancer patients with a pCR while never classifying patients with an incomplete response as having pCR. Importantly, we could validate this finding in three independent datasets, including a prospectively collected cohort. Therefore, this classifier could help select rectal cancer patients for a “watch and wait” strategy.
Translational relevance: Forgoing surgery with its associated side effects could be an option for rectal cancer patients if the prediction of a pathological complete response (pCR) after preoperative chemoradiotherapy would be possible. Based on gene-expression profiles of 161 patients a classifier was developed and validated in three independent datasets (n = 76), identifying over 1/3 of patients with pCR, while never misclassifying a non-complete-responder. Therefore, the classifier can identify patients suited for “watch and wait”.
Tightly regulated and cell-specific NADPH-oxidases (Nox) represent one of the major sources of reactive oxygen species (ROS) signaling molecules that are involved in tissue development and stem cell self-renewal. We have characterized the role of Nox4 in osteo-progenitors during postnatal bone development. Nox4 expression in bone and ROS generation were increased during early osteoblast differentiation and bone development. Stromal osteoblastic cell self-renewal, proliferation and ROS production were significantly lower in samples from whole-body Nox4 knockout mice (Nox4-/-) and conditional knockout (CKO) mice with depletion of Nox4 in the limb bud mesenchyme compared with those from control mice (Nox4fl/fl), but they were reversed after 9 passages. In both sexes, bone volume, trabecular number and bone mineral density were significantly lower in 3-week old CKO and Nox4-/- mice compared with Nox4fl/fl controls. This was reflected in serum levels of bone formation markers alkaline phosphatase (ALP) and procollagen 1 intact N-terminal propeptide (P1NP). However, under-developed bone formation in 3-week old CKO and Nox4-/- mice quickly caught up to levels of control mice by 6-week of age, remained no different at 13-week of age, and was reversed in 32-week old male mice. Osteoclastogenesis showed no differences among groups, however, CTX1 reflecting osteoclast activity was significantly higher in 3-week old male CKO and Nox4-/- mice compared with control mice, and significantly lower in 32-week old Nox4-/- mice compared with control mice. These data suggest that Nox4 expression and ROS signaling in bone and osteoblastic cells coordinately play an important role in osteoblast differentiation, proliferation and maturation.
Das Neuroblastom ist der am häufigsten vorkommende extrakranielle solide Tumor im Kindesalter. Der klinische Verlauf ist sehr heterogen und reicht von spontaner Regression der Erkrankung bis zum Tod trotz intensiver multimodaler Therapie. Vor allem die Prognose des Hochrisiko-Neuroblastoms hat sich in den letzten 20 Jahren kaum verbessert. Nach wie vor versterben 50% der Patient*innen mit Hochrisiko-Neuroblastom trotz intensiver Therapie. Chemoresistenz zählt zu den größten Problemen der heutigen Krebstherapie. In der AG Cinatl am Institut für Medizinische Virologie des Universitätsklinikum Frankfurt existiert eine Sammlung von adaptierten chemoresistenten Tumorzelllinien (Resistant Cancer Cell Line Collection).
Die humane Neuroblastomzelllinie IMR5 und ihre chemoresistenten Sublinien (adaptiert gegen Cisplatin, Gemcitabin, Vincristin und Doxorubicin) wurden in der vorliegenden Arbeit zunächst hinsichtlich der Resistenzmechanismen charakterisiert. In IMR5 wurde durch die Adaption an Vincristin und Doxorubicin eine MDR1-Expression induziert, nicht jedoch durch Adaption an Gemcitabin und Cisplatin. MDR1 vermittelt in IMR5 die Kreuzresistenz zwischen Vincristin und Doxorubicin. Anhand der Daten kann von einer Kreuzresistenz der Vincristin und Doxorubicin adaptierten Sublinien zu anderen MDR1-abhängigen Substraten ausgegangen werden. Verapamil wurde zur Revertierung der MDR1-vermittelten Resistenz verwendet. In der Sequenzierung des TP53-Gens konnte bei allen IMR5-Sublinien Wildtyp-TP53 mit dem Polymorphismus P72R bestätigt und somit TP53-Mutationen als Resistenzmechanismus ausgeschlossen werden. Auch gegen den MDM2-Inhibitor und p53-Aktivator Nutlin-3 zeigen die resistenten Sublinien keine signifikanten Unterschiede. Die Adaptierung hat in keiner Sublinie zu TP53-Mutationen geführt.
Des Weiteren wurde die Wirksamkeit von vier PARP-Inhibitoren (Olaparib, Rucaparib, Niraparib, Talazoparib) in den resistenten IMR5-Sublinien getestet. Talazoparib zeigt sich dabei als der wirksamste PARP-Inhibitor mit IC50-Werten im nM-Bereich. Eine Kreuzresistenz der MDR1-exprimierenden Sublinien IMR5rVCR10 und IMR5rDOXO20 zu den als MDR1-Substraten bekannten PARP-Inhibitoren Olaparib, Rucaparib und Talazoparib konnte ebenfalls nachgewiesen und auf die MDR1-Expression zurückgeführt werden. Auffällig ist die gleich starke Wirksamkeit von Niraparib in allen Sublinien, unabhängig von der Adaption und des MDR1-Status der jeweiligen IMR5-Sublinie. Niraparib ist somit sehr interessant für die klinische Anwendung.
IMR5rCDDP1000 zeigt trotz nur geringer MDR1-Expression eine signifikante Kreuzresistenz gegen Rucaparib und Talazoparib. Verapamil zeigt an IMR5rCDDP1000 keine Veränderung der Wirksamkeit der PARP-Inhibitoren. In IMR5rCDDP1000 ist die Kreuzresistenz gegen PARP-Inhibitoren also MDR1-unabhängig. IMR5rGEMCI20 zeigt dagegen gegen alle PARP-Inhibitoren die gleiche Sensitivität wie IMR5 PAR, gegen Olaparib sogar eine Hypersensitivität.
Die Trinukleotidreduktase SAMHD1 ist als Resistenzmechanismus gegenüber Cytarabin und möglicher Biomarker der AML bekannt. Auch in der Neuroblastomzelllinie IMR5 konnte durch den Abbau von SAMHD1 mit VPX-VLPs die Wirksamkeit des Nukleosidanalogons Cytarabin erhöht werden. Die Wirksamkeit des Nukleosidanalogons Gemcitabin konnte durch den Abbau von SAMHD1 in den IMR5-Sublinien nicht erhöht werden. Eine Beteiligung von SAMHD1 an der Hydrolyse von Gemcitabin konnte somit nicht nachgewiesen werden. Außerdem konnte eine Beteiligung von SAMHD1 an der DNA-Reparatur nicht bestätigt werden: Durch den Abbau von SAMHD1 wurde die Wirksamkeit von DNA-schädigenden Cytostatika wie Topotecan und PARP-Inhibitoren nicht erhöht.
Im Verlauf der Arbeit wurde zusätzlich die Olaparib-resistente Sublinie IMR5rOLAPARIB20 etabliert. Die erworbene Resistenz gegen Olaparib wurde bestätigt, die Zelle zeigt zusätzlich eine MDR1-unabhängige Kreuzresistenz zu Niraparib, Rucaparib und Talazoparib. Die Resistenz beruht wahrscheinlich auf der verminderten Expression von PARP. In IMR5rOLAPARIB20 erhöht der Abbau von SAMHD1 mit VPX-VLPs wie erwartet die Wirksamkeit von Cytarabin, zeigt aber keinen Einfluss auf die Toxizität von PARP-Inhibitoren. Erstaunlicherweise konnte eine erhöhte Wirksamkeit von PARP-Inhibitoren in Kombination mit VPR-VLPs in IMR5 PAR und IMR5rOLAPARIB20 gezeigt werden. PARP-Inhibitoren werden bereits bei soliden Tumoren wie Eierstock- und Brustkrebs eingesetzt und könnten in Kombination mit VPR-VLPs eine weitere Therapiemöglichkeit des Neuroblastoms sein.
The SARS-CoV-2 Omicron variant is currently causing a large number of infections in many countries. A number of antiviral agents are approved or in clinical testing for the treatment of COVID-19. Despite the high number of mutations in the Omicron variant, we here show that Omicron isolates display similar sensitivity to eight of the most important anti-SARS-CoV-2 drugs and drug candidates (including remdesivir, molnupiravir, and PF-07321332, the active compound in paxlovid), which is of timely relevance for the treatment of the increasing number of Omicron patients. Most importantly, we also found that the Omicron variant displays a reduced capability of antagonising the host cell interferon response. This provides a potential mechanistic explanation for the clinically observed reduced pathogenicity of Omicron variant viruses compared to Delta variant viruses.
Recently, we have shown that SARS-CoV-2 Omicron virus isolates are less effective at inhibiting the host cell interferon response than Delta viruses. Here, we present further evidence that reduced interferon-antagonising activity explains at least in part why Omicron variant infections are inherently less severe than infections with other SARS-CoV-2 variants. Most importantly, we here also show that Omicron variant viruses display enhanced sensitivity to interferon treatment, which makes interferons promising therapy candidates for Omicron patients, in particular in combination with other antiviral agents.
Background: Nearly all patients with newly diagnosed glioblastoma experience recurrence following standard-of-care radiotherapy (RT) + temozolomide (TMZ). The purpose of the phase III randomized CheckMate 548 study was to evaluate RT + TMZ combined with the immune checkpoint inhibitor nivolumab (NIVO) or placebo (PBO) in patients with newly diagnosed glioblastoma with methylated MGMT promoter (NCT02667587).
Methods: Patients (N = 716) were randomized 1:1 to NIVO [(240 mg every 2 weeks × 8, then 480 mg every 4 weeks) + RT (60 Gy over 6 weeks) + TMZ (75 mg/m2 once daily during RT, then 150-200 mg/m2 once daily on days 1-5 of every 28-day cycle × 6)] or PBO + RT + TMZ following the same regimen. The primary endpoints were progression-free survival (PFS) and overall survival (OS) in patients without baseline corticosteroids and in all randomized patients.
Results: As of December 22, 2020, median (m)PFS (blinded independent central review) was 10.6 months (95% CI, 8.9-11.8) with NIVO + RT + TMZ vs 10.3 months (95% CI, 9.7-12.5) with PBO + RT + TMZ (HR, 1.1; 95% CI, 0.9-1.3) and mOS was 28.9 months (95% CI, 24.4-31.6) vs 32.1 months (95% CI, 29.4-33.8), respectively (HR, 1.1; 95% CI, 0.9-1.3). In patients without baseline corticosteroids, mOS was 31.3 months (95% CI, 28.6-34.8) with NIVO + RT + TMZ vs 33.0 months (95% CI, 31.0-35.1) with PBO + RT + TMZ (HR, 1.1; 95% CI, 0.9-1.4). Grade 3/4 treatment-related adverse event rates were 52.4% vs 33.6%, respectively.
Conclusions: NIVO added to RT + TMZ did not improve survival in patients with newly diagnosed glioblastoma with methylated or indeterminate MGMT promoter. No new safety signals were observed.
Characteristics of critical incident reporting systems in primary care: an international survey

Aim: The aim of the study was to support the development of future critical incident reporting systems (CIRS) in primary care by collecting information on existing systems. Our focus was on processes used to report and analyse incidents, as well as strategies used to overcome difficulties.
Methods: Based on literature from throughout the world, we identified existing CIRS in primary care. We developed a questionnaire and sent it to operators of a purposeful sample of 17 CIRS in primary care. We used cross-case analysis to compare the answers and pinpoint important similarities and differences in the CIRS in our sample.
Results: Ten CIRS operators filled out the questionnaire, and 9 systems met the inclusion criteria. The sample of CIRS came from 8 different countries and was rather heterogeneous. The reporting systems invited a broad range of professions to report, with some also including reports by patients. In most cases, reporting was voluntary and conducted via an online reporting form. Reports were analysed locally, centrally, or both. The various CIRS used interesting ideas to deal with barriers. Some, for example, used confidential reporting modes as a compromise between anonymity and the need for follow-up investigations, whereas others used smartphone applications and call centres to speed up the reporting process.
Conclusion: We found multiple CIRS that have operated in primary care for many years, have received a high number of reports and were largely developed in accordance with recommendations found in literature. Although primary care in Germany differs from other countries, these CIRS could serve as an inspiration for CIRS in German primary care.
Background: An essential step in any medical research project after identifying the research question is to determine if there are sufficient patients available for a study and where to find them. Pursuing digital feasibility queries on available patient data registries has proven to be an excellent way of reusing existing real-world data sources. To support multicentric research, these feasibility queries should be designed and implemented to run across multiple sites and securely access local data. Working across hospitals usually involves working with different data formats and vocabularies. Recently, the Fast Healthcare Interoperability Resources (FHIR) standard was developed by Health Level Seven to address this concern and describe patient data in a standardized format. The Medical Informatics Initiative in Germany has committed to this standard and created data integration centers, which convert existing data into the FHIR format at each hospital. This partially solves the interoperability problem; however, a distributed feasibility query platform for the FHIR standard is still missing.
Objective: This study described the design and implementation of the components involved in creating a cross-hospital feasibility query platform for researchers based on FHIR resources. This effort was part of a large COVID-19 data exchange platform and was designed to be scalable for a broad range of patient data.
Methods: We analyzed and designed the abstract components necessary for a distributed feasibility query. This included a user interface for creating the query, backend with an ontology and terminology service, middleware for query distribution, and FHIR feasibility query execution service.
Results: We implemented the components described in the Methods section. The resulting solution was distributed to 33 German university hospitals. The functionality of the comprehensive network infrastructure was demonstrated using a test data set based on the German Corona Consensus Data Set. A performance test using specifically created synthetic data revealed the applicability of our solution to data sets containing millions of FHIR resources. The solution can be easily deployed across hospitals and supports feasibility queries, combining multiple inclusion and exclusion criteria using standard Health Level Seven query languages such as Clinical Quality Language and FHIR Search. Developing a platform based on multiple microservices allowed us to create an extendable platform and support multiple Health Level Seven query languages and middleware components to allow integration with future directions of the Medical Informatics Initiative.
Conclusions: We designed and implemented a feasibility platform for distributed feasibility queries, which works directly on FHIR-formatted data and distributed it across 33 university hospitals in Germany. We showed that developing a feasibility platform directly on the FHIR standard is feasible.
The advancement of medical technology has led not only to an increase in life expectancy but also to a rise in aging-related diseases. Aging promotes metabolic disorders, in turn affecting cardiovascular health. Derailment of biological processes in the pancreas, liver, adipose tissue, and skeletal muscle impairs glucose and lipid metabolism, and mitochondrial function, triggering the development of diabetes and lipid-related disorders that inflict damage on cardiac and vascular tissues. Long noncoding RNAs (lncRNAs) regulate a wide range of biological process and are one of the key factors controlling metabolism and mitochondria. Here, we discuss the versatile function of lncRNAs involved in the metabolic regulation of glucose and lipid, and mitochondrial function, and how the dysregulation of lncRNAs induces the development of various metabolic disorders and their cardiovascular consequences.
Background: Trauma may be associated with significant to life-threatening blood loss, which in turn may increase the risk of complications and death, particularly in the absence of adequate treatment. Hydroxyethyl starch (HES) solutions are used for volume therapy to treat hypovolemia due to acute blood loss to maintain or re-establish hemodynamic stability with the ultimate goal to avoid organ hypoperfusion and cardiovascular collapse. The current study compares a 6% HES 130 solution (Volulyte 6%) versus an electrolyte solution (Ionolyte) for volume replacement therapy in adult patients with traumatic injuries, as requested by the European Medicines Agency to gain more insights into the safety and efficacy of HES in the setting of trauma care.
Methods: TETHYS is a pragmatic, prospective, randomized, controlled, double-blind, multicenter, multinational trial performed in two parallel groups. Eligible consenting adults ≥ 18 years, with an estimated blood loss of ≥ 500 ml, and in whom initial surgery is deemed necessary within 24 h after blunt or penetrating trauma, will be randomized to receive intravenous treatment at an individualized dose with either a 6% HES 130, or an electrolyte solution, for a maximum of 24 h or until reaching the maximum daily dose of 30 ml/kg body weight, whatever occurs first. Sample size is estimated as 175 patients per group, 350 patients total (α = 0.025 one-tailed, power 1–β = 0.8). Composite primary endpoint evaluated in an exploratory manner will be 90-day mortality and 90-day renal failure, defined as AKIN stage ≥ 2, RIFLE injury/failure stage, or use of renal replacement therapy (RRT) during the first 3 months. Secondary efficacy and safety endpoints are fluid administration and balance, changes in vital signs and hemodynamic status, changes in laboratory parameters including renal function, coagulation, and inflammation biomarkers, incidence of adverse events during treatment period, hospital, and intensive care unit (ICU) length of stay, fitness for ICU or hospital discharge, and duration of mechanical ventilation and/or RRT.
Discussion: This pragmatic study will increase the evidence on safety and efficacy of 6% HES 130 for treatment of hypovolemia secondary to acute blood loss in trauma patients.

Background: Blood pressure is known to be increased in kidney donors following living-donor kidney transplantation. However, the physiological underpinnings of the blood-pressure increase following uninephrectomy remain unclear. We hypothesized that changes in sympathetic tone or in parasympathetic modulation of sinus node function are involved in the blood-pressure increase following experimental kidney-mass reduction.
Methods: C57BL6N mice (6 to 11 per group) subjected to sham surgery (controls) or uninephrectomy with or without a one-week course of sodium chloride-enriched, taurine-deficient diet were studied. Uninephrectomized mice treated with a subcutaneous infusion of angiotensin-II over a period of one week were positive controls. A transfemoral aortic catheter with telemetry unit was implanted, readings of heart-rate and blood-pressure were recorded. Powerspectral analysis of heart rate and systolic blood pressure was performed to gain surrogate parameters of sympathetictone and parasympathetic modulation of sinus node function. Baroreflex sensitivity of heart rate was determined from awake, unrestrained mice using spontaneous baroreflex gain technique.
Results: Systolic arterial blood pressure, heart rate and baroreflex sensitivity were not different in uninephrectomized mice when compared to controls. Parasympathetic modulation of sinus node function was less in uninephrectomized mice in comparison to controls. Uninephrectomized mice of the high-angiotensin-II model or of the high-salt and taurine-deficiency model had an increased systolic arterial blood pressure.
Conclusions: Uninephrectomy associated with less parasympathetic modulation of sinus node function. The combination of uninephrectomy, taurine-deficiency and high-salt intake led to arterial hypertension.
Nowadays, several options are available to treat patients with conductive or mixed hearing loss. Whenever surgical intervention is not possible or contra-indicated, and amplification by a conventional hearing device (e.g., behind-the-ear device) is not feasible, then implantable hearing devices are an indispensable next option. Implantable bone-conduction devices and middle-ear implants have advantages but also limitations concerning complexity/invasiveness of the surgery, medical complications, and effectiveness. To counsel the patient, the clinician should have a good overview of the options with regard to safety and reliability as well as unequivocal technical performance data. The present consensus document is the outcome of an extensive iterative process including ENT specialists, audiologists, health-policy scientists, and representatives/technicians of the main companies in this field. This document should provide a first framework for procedures and technical characterization to enhance effective communication between these stakeholders, improving health care.
Activated SUMOylation restricts MHC class I antigen presentation to confer immune evasion in cancer

Activated SUMOylation is a hallmark of cancer. Starting from a targeted screening for SUMO-regulated immune evasion mechanisms, we identified an evolutionarily conserved function of activated SUMOylation, which attenuated the immunogenicity of tumor cells. Activated SUMOylation allowed cancer cells to evade CD8+ T cell–mediated immunosurveillance by suppressing the MHC class I (MHC-I) antigen-processing and presentation machinery (APM). Loss of the MHC-I APM is a frequent cause of resistance to cancer immunotherapies, and the pharmacological inhibition of SUMOylation (SUMOi) resulted in reduced activity of the transcriptional repressor scaffold attachment factor B (SAFB) and induction of the MHC-I APM. Consequently, SUMOi enhanced the presentation of antigens and the susceptibility of tumor cells to CD8+ T cell–mediated killing. Importantly, SUMOi also triggered the activation of CD8+ T cells and thereby drove a feed-forward loop amplifying the specific antitumor immune response. In summary, we showed that activated SUMOylation allowed tumor cells to evade antitumor immunosurveillance, and we have expanded the understanding of SUMOi as a rational therapeutic strategy for enhancing the efficacy of cancer immunotherapies.
Although direct-acting antiviral medications effectively cure hepatitis C in most patients, sometimes treatment selects for resistant viruses, causing antiviral drugs to be either ineffective or only partially effective. Multidrug resistance is common in patients for whom DAA treatment fails. Older patients and patients with advanced liver diseases are more likely to select drug-resistant viruses. Collective efforts from international communities and governments are needed to develop an optimal approach to managing drug resistance and preventing the transmission of resistant viruses.
The induction of apoptosis is a direct way to eliminate tumor cells and improve cancer therapy. Apoptosis is tightly controlled by the balance of pro- and antiapoptotic Bcl-2 proteins. BH3 mimetics neutralize the antiapoptotic function of Bcl-2 proteins and are highly promising compounds inducing apoptosis in several cancer entities including pediatric malignancies. However, the clinical application of BH3 mimetics in solid tumors is impeded by the frequent resistance to single BH3 mimetics and the anticipated toxicity of high concentrations or combination treatments. One potential avenue to increase the potency of BH3 mimetics is the development of immune cell-based therapies to counteract the intrinsic apoptosis resistance of tumor cells and sensitize them to immune attack. Here, we describe spheroid cultures of pediatric cancer cells that can serve as models for drug testing. In these 3D models, we were able to demonstrate that activated allogeneic Natural Killer (NK) cells migrated into tumor spheroids and displayed cytotoxicity against a wide range of pediatric cancer spheroids, highlighting their potential as anti-tumor effector cells. Next, we investigated whether treatment of tumor spheroids with subtoxic concentrations of BH3 mimetics can increase the cytotoxicity of NK cells. Notably, the cytotoxic effects of NK cells were enhanced by the addition of BH3 mimetics. Treatment with either the Bcl-XL inhibitor A1331852 or the Mcl-1 inhibitor S63845 increased the cytotoxicity of NK cells and reduced spheroid size, while the Bcl-2 inhibitor ABT-199 had no effect on NK cell-mediated killing. Taken together, this is the first study to describe the combination of BH3 mimetics targeting Bcl-XL or Mcl-1 with NK cell-based immunotherapy, highlighting the potential of BH3 mimetics in immunotherapy.
Bei fehlender Milz nach einer totalen Splenektomie kann es unter anderem zum lebensgefährlichen Postsplenektomie-Syndrom (overwhelming postplenectomy infection, OPSI) kommen, weshalb die partielle Splenektomie (PS) zunehmend in den Fokus rückt. Die Reduktion perioperativer Komplikationen erscheint zudem wünschenswert, weswegen der laparoskopische Operationsansatz zunehmende medizinische Resonanz genießt. Letzteren verbinden Chirurgen häufig mit geringeren postoperativen Schmerzen für den Patienten, besseren kosmetischen Ergebnissen und einer Reduktion chirurgischer Komplikationen.
Ziel dieser Studie war es, anhand verschiedener Parameter die perioperativen Ergebnisse zwischen der laparoskopischen partiellen Splenektomie (LPS) und der offenen partiellen Splenektomie (OPS) bei Kindern und Jugendlichen zu vergleichen. Betrachtet wurde ein Zeitraum von etwa zehn Jahren (2008-2018), wobei insgesamt 26 Patienten die Einschlusskriterien erfüllten. 10 Patienten unterzogen sich der LPS, während bei 16 Patienten eine OPS durchgeführt wurde. Anhand der digitalen Patientenakten wurden verschiedene perioperative Parameter erhoben, welche unter anderem die postoperative Schmerzstärke, den Analgetikabedarf, verschiedene hämatologische Variablen sowie die perioperativen Komplikationen umfassten.
Zwischen den beiden Gruppen unterschieden sich das postoperative Schmerzbefinden mit entsprechend analgetischer Therapie, der zeitliche Beginn des oralen Kostaufbaus und der postoperativen Mobilisation sowie die Dauer des stationären Aufenthalts nicht signifikant. Ferner wurden die chirurgischen Komplikationen anhand der Clavien-Dindo-Klassifikation (CDC) in verschiedene Grade eingeteilt und konnten sowohl in der laparoskopischen als auch in der offen-chirurgisch operierten Gruppe zeigen, dass hauptsächlich leichte Komplikationen vorlagen, bei denen ein Großteil der Patienten lediglich eine medikamentöse Therapie (80.8%) für die aufgetretenen Komplikationen (CDC ≤ II) benötigte. Lediglich bei 13% der offen operierten Patienten bedurfte es einer chirurgischen bzw. endoskopischen Intervention (CDC ≥ III).
Die Operationsdauer zwischen den beiden Gruppen unterschied sich jedoch signifikant. Bei der partiellen Splenektomie nahm der laparoskopische Zugangsweg signifikant mehr Zeit in Anspruch als bei der offen-chirurgisch operierten Gruppe. Darüber hinaus wurde der perioperative Blutverlust anhand der Mercuriali-Formel berechnet und zeigte bei der LPS-Gruppe einen signifikant höheren perioperativen Blutverlust als bei der OPS.
Die durch diese Studie erzielten Resultate zeigen anhand verschiedener perioperativer Parameter, dass die LPS im Vergleich zur etablierten OPS trotz signifikanter Unterschiede in der Operationszeit und dem perioperativen Blutverlust beide sichere und praktikable Operationsverfahren sind. Zusammenfassend stellten sich hinsichtlich der weiteren perioperativen Ergebnisse keine signifikanten Unterschiede zwischen beiden Gruppen dar.
Im Rahmen unserer Untersuchung konnte zudem dargestellt werden, dass die postoperativen Rekonvaleszenzvariablen sowie die Scores zur Erfassung der chirurgischen Komplikationen keinen signifikanten Unterschied ergaben. Ebenso wies die postoperativ dokumentierte Schmerzstärke der Patienten und der daraus resultierende Analgetikabedarf keine signifikanten Unterschiede auf.
Mutations of the isocitrate dehydrogenase-1 (IDH1) and IDH2 genes are among the most frequent alterations in acute myeloid leukemia (AML) and can be found in ∼20% of patients at diagnosis. Among 4930 patients (median age, 56 years; interquartile range, 45-66) with newly diagnosed, intensively treated AML, we identified IDH1 mutations in 423 (8.6%) and IDH2 mutations in 575 (11.7%). Overall, there were no differences in response rates or survival for patients with mutations in IDH1 or IDH2 compared with patients without mutated IDH1/2. However, distinct clinical and comutational phenotypes of the most common subtypes of IDH1/2 mutations could be associated with differences in outcome. IDH1-R132C was associated with increased age, lower white blood cell (WBC) count, less frequent comutation of NPM1 and FLT3 internal tandem mutation (ITD) as well as with lower rate of complete remission and a trend toward reduced overall survival (OS) compared with other IDH1 mutation variants and wild-type (WT) IDH1/2. In our analysis, IDH2-R172K was associated with significantly lower WBC count, more karyotype abnormalities, and less frequent comutations of NPM1 and/or FLT3-ITD. Among patients within the European LeukemiaNet 2017 intermediate- and adverse-risk groups, relapse-free survival and OS were significantly better for those with IDH2-R172K compared with WT IDH, providing evidence that AML with IDH2-R172K could be a distinct entity with a specific comutation pattern and favorable outcome. In summary, the presented data from a large cohort of patients with IDH1/2 mutated AML indicate novel and clinically relevant findings for the most common IDH mutation subtypes.
Objective: To assess the current medical practice in Europe regarding prenatal dexamethasone (Pdex) treatment of congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency.
Design and methods: A questionnaire was designed and distributed, including 17 questions collecting quantitative and qualitative data. Thirty-six medical centres from 14 European countries responded and 30 out of 36 centres were reference centres of the European Reference Network on Rare Endocrine Conditions, EndoERN.
Results: Pdex treatment is currently provided by 36% of the surveyed centres. The treatment is initiated by different specialties, that is paediatricians, endocrinologists, gynaecologists or geneticists. Regarding the starting point of Pdex, 23% stated to initiate therapy at 4–5 weeks postconception (wpc), 31% at 6 wpc and 46 % as early as pregnancy is confirmed and before 7 wpc at the latest. A dose of 20 µg/kg/day is used. Dose distribution among the centres varies from once to thrice daily. Prenatal diagnostics for treated cases are conducted in 72% of the responding centres. Cases treated per country and year vary between 0.5 and 8.25. Registries for long-term follow-up are only available at 46% of the centres that are using Pdex treatment. National registries are only available in Sweden and France.
Conclusions: This study reveals a high international variability and discrepancy in the use of Pdex treatment across Europe. It highlights the importance of a European cooperation initiative for a joint international prospective trial to establish evidence-based guidelines on prenatal diagnostics, treatment and follow-up of pregnancies at risk for CAH.
Myocardial injury as induced by myocardial infarction results in tissue ischemia, which critically incepts cardiomyocyte death. Endothelial cells play a crucial role in restoring oxygen and nutrient supply to the heart. Latest advances in single-cell multi-omics, together with genetic lineage tracing, reveal a transcriptional and phenotypical adaptation to the injured microenvironment, which includes alterations in metabolic, mesenchymal, hematopoietic and pro-inflammatory signatures. The extent of transition in mesenchymal or hematopoietic cell lineages is still debated, but it is clear that several of the adaptive phenotypical changes are transient and endothelial cells revert back to a naïve cell state after resolution of injury responses. This resilience of endothelial cells to acute stress responses is important for preventing chronic dysfunction. Here, we summarize how endothelial cells adjust to injury and how this dynamic response contributes to repair and regeneration. We will highlight intrinsic and microenvironmental factors that contribute to endothelial cell resilience and may be targetable to maintain a functionally active, healthy microcirculation.
Besides transcription, RNA decay accounts for a large proportion of regulated gene expression and is paramount for cellular functions. Classical RNA surveillance pathways, like nonsense-mediated decay (NMD), are also implicated in the turnover of non-mutant transcripts. Whereas numerous protein factors have been assigned to distinct RNA decay pathways, the contribution of long non-coding RNAs (lncRNAs) to RNA turnover remains unknown. Here we identify the lncRNA CALA as a potent regulator of RNA turnover in endothelial cells. We demonstrate that CALA forms cytoplasmic ribonucleoprotein complexes with G3BP1 and regulates endothelial cell functions. A detailed characterization of these G3BP1-positive complexes by mass spectrometry identifies UPF1 and numerous other NMD factors having cytoplasmic G3BP1-association that is CALA-dependent. Importantly, CALA silencing impairs degradation of NMD target transcripts, establishing CALA as a non-coding regulator of RNA steady-state levels in the endothelium.
Bei Vorliegen eines metastasierten kastrationsresistenten Prostatakarzinoms ist die Radioligandentherapie mit 177Lu-PSMA-617 bzw. 177Lu-PSMA-I&T (hier abgekürzt 177Lu-PSMA) eine mögliche Therapieoption. Sie setzt eine ausreichende Expression von prostataspezifischem Membranantigen (PSMA) auf dem Tumorgewebe des Prostatakarzinoms voraus. Da PSMA auch auf physiologischem Gewebe exprimiert wird, sind bestimmte Organe einer erhöhten Dosis ausgesetzt. Die Niere ist aufgrund der PSMA-Expression und der renalen Eliminierung des Radionuklids eines dieser Risikoorgane.
Eine generelle Leitlinie zur Überwachung der Nierendosis unter Therapie mit 177Lu-PSMA existiert noch nicht, jedoch wird empfohlen, bei Durchführung mehrerer Therapiezyklen eine Dosimetrie der Risikoorgane durchzuführen. Meist erfolgt die Nierendosimetrie mittels SPECT/CT. Dieses Verfahren wird als Goldstandard angesehen, da es eine Korrektur der Schwächung, Streuung und des Hintergrundes zulässt.
Die Dosimetrie durch eine SPECT/CT-Bildgebung ist jedoch nicht immer möglich. Dementsprechend gibt es in Studien mehrere Ansätze der Nierendosimetrie ohne SPECT/CT.
In dieser Arbeit werden zwei unterschiedliche Verfahren zur Nierendosimetrie vorgestellt, die einzig mit konventionellen nuklearmedizinischen Modalitäten durchgeführt werden. Diese werden an einem großen Patientenkollektiv angewendet und anhand bereits in der Literatur bestehenden Studien und Ergebnissen evaluiert und mit diesen verglichen. Die Nierendosis in der Literatur bewegt sich dabei meist zwischen 0,4 und 0,8 Gy pro GBq applizierter Aktivität (Gy/GBq).
Die hier vorgestellte Methode 1 arbeitet mit Messwerten aus der Gammasonde, Ganzkörperszintigraphie und abdominalem SPECT. Messwerte werden an der Gammasonde 0 und 48 Stunden nach Applikation von 177Lu-PSMA erhoben, die Szintigraphie wird 48 Stunden und die SPECT 24, 48 und 72 Stunden post iniectionem angefertigt.
Methode 2 der Nierendosimetrie dagegen benötigt nur die 24, 48 und 72 Stunden nach Applikation durchgeführte SPECT sowie die durch eine CT-Bildgebung messbaren Abstände der Nieren zur Körperoberfläche. Durch zuvor durchgeführte Messungen mit einem Torso-Phantom ist die Bestimmung einer patientenindividuellen Effektivität möglich, die die Bestimmung der Nierendosis in Abhängigkeit zur Lage der Nieren möglich macht.
Die gemittelte Nierendosis von 196 Patienten, die eine Therapie mit 177Lu-PSMA erhielten, betrugt 0,29 ± 0,14 Gy/GBq bei Methode 1 und 0,54 ± 0,2g Gy/GBq bei Methode 2. Im Schnitt wurden 3,54 ± 2,63 Therapiezyklen je Patient durchgeführt.
Da bisher nur Studien mit einer kleinen Patientenkohorte vorliegen, besteht die Notwendigkeit zur Durchführung von Arbeiten mit mehr Patienten. Die vorliegende Arbeit schließt 196 Patienten mit 926 durchgeführten Therapiezyklen ein, wodurch Ergebnisse erzielt wurden, die valide Aussagen treffen lassen.
Nach Vergleich der Ergebnisse mit bereits vorliegenden Studien sollte Methode 1 der Nierendosimetrie angesichts der fehlenden Möglichkeit zur Schwächungskorrektur verworfen werden, da dies die Gefahr der Unterschätzung der Nierendosis mit sich bringt. Stattdessen sollte Methode 2 präferiert werden, deren Ergebnisse sich mit den anderen zum Vergleich herangezogenen Studien decken.
Eine Durchführung mit Methode 2 ist dementsprechend sicher und ist in Betracht zu ziehen, sollte die Nierendosimetrie mit einem SPECT/CT nicht möglich sein.
Providing an interactive undergraduate elective on safety culture online – concept and evaluation

Background: The COVID-19 pandemic has made it more difficult to maintain high quality in medical education. As online formats are often considered unsuitable, interactive workshops and seminars have particularly often been postponed or cancelled. To meet the challenge, we converted an existing interactive undergraduate elective on safety culture into an online event. In this article, we describe the conceptualization and evaluation of the elective.
Methods: The learning objectives of the safety culture elective remained unchanged, but the teaching methods were thoroughly revised and adapted to suit an online setting. The online elective was offered as a synchronous two-day course in winter semester 2020/21 during the “second wave” of the COVID-19 pandemic in Germany. At the end of each day, participating students evaluated the elective by completing an online survey. Items were rated on a six-point Likert scale. We used SPSS for data analysis.
Results: Twenty medical undergraduates completed the elective and rated it extremely positively (1.1 ± 0.2). Students regard safety culture as very important and felt the learning objectives had been achieved. Moreover, they were very satisfied with the design and content of the elective, and especially with interactive elements like role-play. Around 55% of participants would recommend continuing to offer the online elective after the pandemic.
Conclusions: It makes sense to offer undergraduate medical students online elective courses on safety culture, especially during a pandemic. The elective described here can serve as a best practice example of how to teach safety culture to undergraduates, especially when physical presence is unfeasible. Electives requiring a high degree of interaction can also function well online.
(1) Background: Patients with locally advanced head and neck squamous cell carcinoma (HNSCC) who are biologically at high risk for the development of loco–regional recurrences after postoperative radiotherapy (PORT) but at intermediate risk according to clinical risk factors may benefit from additional concurrent chemotherapy. In this matched-pair study, we aimed to identify a corresponding predictive gene signature. (2) Methods: Gene expression analysis was performed on a multicenter retrospective cohort of 221 patients that were treated with postoperative radiochemotherapy (PORT-C) and 283 patients who were treated with PORT alone. Propensity score analysis was used to identify matched patient pairs from both cohorts. From differential gene expression analysis and Cox regression, a predictive gene signature was identified. (3) Results: 108 matched patient pairs were selected. We identified a 2-metagene signature that stratified patients into risk groups in both cohorts. The comparison of the high-risk patients between the two types of treatment showed higher loco–regional control (LRC) after treatment with PORT-C (p < 0.001), which was confirmed by a significant interaction term in Cox regression (p = 0.027), i.e., the 2-metagene signature was indicative for the type of treatment. (4) Conclusion: We have identified a novel gene signature that may be helpful to identify patients with high-risk HNSCC amongst those at intermediate clinical risk treated with PORT, who may benefit from additional concurrent chemotherapy.
Die Niere ist das in Deutschland am häufigsten transplantierte Organ. Patienten nach Nierentransplantation haben aufgrund der notwendigen Immunsuppression ein erhöhtes Risiko für Infektionen. Die häufigste Infektion nach einer Nierentransplantation ist die Harnwegsinfektion, eine lebensbedrohliche Verlaufsform mit Entwicklung von Organdysfunktionen bezeichnet man als Urosepsis. Durch die Immunsuppression kann die Erkennung des septischen Krankheitsbildes erheblich erschwert sein, da typische körperliche Reaktionen auf eine Infektion unter Immunsuppression (z.B. Fieber, Tachykardie) verändert ablaufen.
Im Rahmen einer Sepsis laufen parallel pro- und antiinflammatorische Prozesse ab. Ein Überwiegen der antiinflammatorischen Prozesse kann zur Entwicklung einer „Sepsis-induzierten Immunsuppression“ führen, welche ein deutliches Mortalitätsrisiko darstellt, da sie zur fehlenden Sanierung des Infektionsfokus sowie zur Entwicklung sekundärer (z.T. opportunistischer) Infektionen führen kann. Ein zentraler antiinflammatorischer Prozess ist die Apoptose von CD4 und CD8 Lymphozyten. Die Möglichkeit der therapeutischen Beeinflussung dieses immunsupprimierten Zustandes ist aktuell Gegenstand zahlreicher Studien. Ziele der vorliegenden Arbeit waren die Überprüfung, ob sich der Abfall der Lymphoyztenzahl/eine Lymphopenie im Rahmen einer Infektionsepsiode auch in der immunsupprimierten Patientenklientel nach Nierentranplantation darstel-len lässt. Des Weiteren sollte diskutiert werden, ob der Nachweis einer reduzierten CD4/8 Lymphozytenzahl geeignet sein kann, diagnostisch die Infektionssituation einzuschätzen und ggf. die immunsuppressive Therapie anzupassen.
In dieser retrospektiven Kohortenanalyse wurden die Daten von nierentransplantierten Patienten ausgewertet, welche in den Jahren 2010 und 2011 im Universitätsklinikum der Goethe-Universität Frankfurt am Main aufgrund einer Harnwegsinfektion (n=47) oder Urosepsis (n=75) behandelt wurden. Es erfolgte ein Ver-gleich der CD4/CD8 Lymphozytenzahl vor, während und nach einer Infektions-episode. Ergänzend wurden die Häufigkeit einer CMV-Reaktivierung/-Infektion sowie das ursächliche bakterielle Erregerspektrum mit zugehörigem Antibiotika-resistenzmuster erhoben.
Sowohl Patienten mit Urosepsis als auch mit Harnwegsinfektion zeigten einen signifikanten Abfall der CD4 Lymphozyten im Rahmen der Infektionsepisode (p<0,01; p<0,05). Patienten mit Urosepsis zeigten zudem einen signifikanten Abfall der CD8 Lymphozyten (p<0,01). Patienten mit Erregernachweis in der Blutkultur zeigten signifikant niedrigere CD4/8 Lymphozyten während der Infek-tionsepisode (p<0,05) als Patienten mit negativer Blutkultur. Eine niedrige CD4/8 Lymphozytenzahl korrelierte besser mit einem Erregernachweis mittels Blutkultur als das Erfüllen der SIRS Kriterien. Mit Behandlung der Infektionsepi-sode zeigte sich ein Wiederanstieg der CD4/8 Lymphozytenzahlen. Unter Re-duktion/Pausieren der immunsuppressiven Therapie kam es zu keiner Abstoßungsreaktion/keinem Versterben eines Patienten im Rahmen der Infektionsepisode. Zu einem CMV-DNA Nachweis kam es bei 15 Patienten (20%) der Gruppe Urosepsis sowie 8 Patienten (17%) der Gruppe Harnwegsinfektion während der Infektionsepisode. Sowohl in Urin- als auch Blutkulturen waren E.coli gefolgt von Enterokokken die häufigsten nachgewiesenen Erreger. E.coli fanden sich signifikant häufiger in Urin- und Blutkulturen bei Urosepsis, Enterokokken signifikant häufiger in Urinkulturen bei Harnwegsinfektionen. Es zeigten sich hohe Antibiotikaresistenzraten insbesondere gegenüber Cotrimoxazol (59% der E.coli) und Fluorchinolonen (45% der E.coli, 82% der Enterokokken). 11% der E.coli und 62% der Klebsiellen waren als 3MRGN Erreger zu klassifizieren, 15% der Klebsiellen als 4MRGN mit Carbapenemresistenz.
Zusammenfassend lässt sich auch bei immunsupprimier-ten/nierentransplantierten Patienten im Rahmen einer Infektionsepisode ein signifikanter Abfall der CD4/8 Lymphozyten nachweisen. Das Reduzieren/Pausieren der immunsupressiven Therapie erscheint anhand der vorgelegten Daten sinnvoll, ein Monitoring der CD4/8 Lymphozytenzahl könnte zur Therapiesteuerung geeignet sein. Im Rahmen weiterer prospektiver Studien könnte eine Erarbeitung eines festen Schemas der Anpassung der immunsuppressiven Therapie an eine bestimmte CD4/8 Lymphozytenzahl sinnvoll sein. Passend zur Vergleichsliteratur waren E.coli und Enterokokken die am häufigsten nachgewiesenen Erreger. Die hohen Antibiotikaresistenzmuster unterstreichen die Notwendigkeit, das lokale Erregermilieu zu kennen, um möglichst passend kalkuliert antibiotisch behandeln zu können.
Depressive symptoms in youth with ADHD: the role of impairments in cognitive emotion regulation

Youth with attention-deficit/hyperactivity disorder (ADHD) are at increased risk to develop co-morbid depression. Identifying factors that contribute to depression risk may allow early intervention and prevention. Poor emotion regulation, which is common in adolescents, is a candidate risk factor. Impaired cognitive emotion regulation is a fundamental characteristic of depression and depression risk in the general population. However, little is known about cognitive emotion regulation in youth with ADHD and its link to depression and depression risk. Using explicit and implicit measures, this study assessed cognitive emotion regulation in youth with ADHD (N = 40) compared to demographically matched healthy controls (N = 40) and determined the association with depressive symptomatology. As explicit measure, we assessed the use of cognitive emotion regulation strategies via self-report. As implicit measure, performance in an ambiguous cue-conditioning task was assessed as indicator of affective bias in the processing of information. Compared to controls, patients reported more frequent use of maladaptive (i.e., self-blame, catastrophizing, and rumination) and less frequent use of adaptive (i.e., positive reappraisal) emotion regulation strategies. This pattern was associated with the severity of current depressive symptoms in patients. In the implicit measure of cognitive bias, there was no significant difference in response of patients and controls and no association with depression. Our findings point to depression-related alterations in the use of cognitive emotion regulation strategies in youth with ADHD. The study suggests those alterations as a candidate risk factor for ADHD-depression comorbidity that may be used for risk assessment and prevention strategies.
Ziel unserer retrospektiven Studie war es, die refraktiven und kornealen Veränderungen nach DMEK bei pseudophaken Patienten, die sich auf Grund einer endothelialen Hornhauterkrankung behandeln ließen, zu untersuchen. Durch unsere einheitlich pseudophake Patientenkohorte wollten wir untersuchen, ob sich die refraktiven Veränderungen nach DMEK von den bereits bekannten Änderungen bei einer simultan durchgeführten Katarakt- und DMEK-Operation sogenannte „Triple“-DMEK unterscheiden. Primärer Endpunkt der Studie war die Veränderung der Refraktion unter besonderer Berücksichtigung des sphärischen Äquivalents (SEQ) des jeweiligen pseudophaken Auges nach DMEK. Sekundäre Endpunkte umfassten die Entwicklung des Visus, der CCT, der ECD und verschiedener kornealer Parameter, die mittels Scheimpflug- Tomographie ermittelt wurden.
In der vorliegenden Arbeit erfolgte hierzu die retrospektive Analyse von Daten, die in den Patientenakten dokumentiert und digital gespeichert waren (Pentacam® HR). Es wurden 109 Augen von 95 Patienten, die sich im Zeitraum von Februar 2015 bis Dezember 2018 mittels DMEK in unserem Zentrum behandeln ließen, in die Studie eingeschlossen. Davon stammten 66 Augen (61%) von weiblichen und 43 Augen (39%) von männlichen Patienten. Es handelte sich bei 61 Augen (56%) um ein linkes und bei 48 Augen (44 %) um ein rechtes Auge. Die Patienten waren 20 bis 91 Jahre alt. Das mittlere Alter zum Zeitpunkt der DMEK-Operation betrug 71,9 Jahre (SD ±10,23). Der Altersmittelwert der Männer lag bei 70,4 Jahren (SD 11,23, Spannweite: 20-84) und der der Frauen bei 72,9 Jahren (SD 9,49, Spannweite: 47-91). Der mittlere Nachbeobachtungszeitraum betrug 10,47 Monate (SD 6,78, Spannweite: 1-28 Monate).
Für das SEQ konnte bei Betrachtung aller ausgewerteten Daten eine leichte Tendenz in Richtung eines „hyperopen shifts“ mit einer mittleren Veränderung des SEQ von + 0,1 D gezeigt werden, die jedoch nicht statistisch signifikant war. Zwischenzeitlich kam es im Nachbeobachtungszeitraum zu einer klaren Tendenz hinsichtlich einer Myopisierung bei Betrachtung aller ausgewerteten Daten. Der „hyperope shift“ konnte erst am Ende, möglicherweise infolge einer deutlich reduzierten Patientenzahl, beobachtet werden. In der Subgruppe „vollständige Kontrollen“ für das SEQ zeigte sich eine Tendenz hinsichtlich einer leichten Abnahme des SEQ, die jedoch ebenfalls nicht statistisch signifikant war und aufgrund der geringen Gruppengröße (n=32) kritisch betrachtet werden sollte. In unserer Kohorte konnte somit keine eindeutige Aussage über eine postoperative Änderung des SEQ in Richtung eines „hyperopen shifts“ oder „myopen shifts“ gemacht werden. Die Refraktion verhielt sich in unserem Patientenkollektiv nach DMEK insgesamt weitestgehend stabil. Insofern sind refraktive "Überraschungen", wie sie weiterhin häufig nach "Triple"-DMEK zu beobachten sind, bei zuvor pseudophakisierten Patienten in einem weitaus geringeren Maße zu erwarten. Unter den mittels Scheimpflug-Technologie untersuchten kornealen Parametern wies lediglich der posteriore Astigmatismus signifikante Veränderungen im Sinne einer Reduktion der kornealen Krümmung auf. Indirekt heben unsere Ergebnisse damit die Bedeutung des posterioren Hornhautprofils auf die postoperative Refraktionsentwicklung und somit auch auf die IOL-Kalkulation bei "Triple"-DMEK-Prozeduren hervor. Außerdem scheint der postoperative Anstieg der Sehschärfe mit den gleichfalls signifikanten Änderungen der kornealen Densitometrie in der 2-6 mm Zone, des Hornhautvolumens und der zentralen Hornhautdicke umgekehrt korreliert zu sein.
Darüber hinaus lässt sich festhalten, dass auch unsere Untersuchung in einem gewissen Maße die Überlegenheit der DMEK gegenüber etablierten Techniken, wie beispielsweise der PK verdeutlicht. Sowohl anhand der refraktiven Stabilität als auch der Visusergebnisse konnten wir belegen, dass das DMEK-Verfahren nach der anfänglichen Lernkurve die wohl besten funktionellen Ergebnisse in der Behandlung von Patienten mit endothelialen Hornhauterkrankungen liefert. Besonders bei bereits pseudophaken Patienten weist die DMEK durch die zu erwartende hohe postoperative refraktive Stabilität viele Vorteile auf
und erscheint insbesondere hinsichtlich der Vorhersagbarkeit des postoperativen refraktiven Ergebnisses der „Triple“- DMEK überlegen.
Purpose: To investigate the diagnostic performance of noise-optimized virtual monoenergetic images (VMI+) in dual-energy CT (DECT) of portal vein thrombosis (PVT) compared to standard reconstructions. Method: This retrospective, single-center study included 107 patients (68 men; mean age, 60.1 ± 10.7 years) with malignant or cirrhotic liver disease and suspected PVT who had undergone contrast-enhanced portal-phase DECT of the abdomen. Linearly blended (M_0.6) and virtual monoenergetic images were calculated using both standard VMI and noise-optimized VMI+ algorithms in 20 keV increments from 40 to 100 keV. Quantitative measurements were performed in the portal vein for objective contrast-to-noise ratio (CNR) calculation. The image series showing the greatest CNR were further assessed for subjective image quality and diagnostic accuracy of PVT detection by two blinded radiologists. Results: PVT was present in 38 subjects. VMI+ reconstructions at 40 keV revealed the best objective image quality (CNR, 9.6 ± 4.3) compared to all other image reconstructions (p < 0.01). In the standard VMI series, CNR peaked at 60 keV (CNR, 4.7 ± 2.1). Qualitative image parameters showed the highest image quality rating scores for the 60 keV VMI+ series (median, 4) (p ≤ 0.03). The greatest diagnostic accuracy for the diagnosis of PVT was found for the 40 keV VMI+ series (sensitivity, 96%; specificity, 96%) compared to M_0.6 images (sensitivity, 87%; specificity, 92%), 60 keV VMI (sensitivity, 87%; specificity, 97%), and 60 keV VMI+ reconstructions (sensitivity, 92%; specificity, 97%) (p ≤ 0.01). Conclusions: Low-keV VMI+ reconstructions resulted in significantly improved diagnostic performance for the detection of PVT compared to other DECT reconstruction algorithms.
Hintergrund. Patienten mit Gliomen, die das Corpus callosum miteinbeziehen, befinden sich häufig in einem schlechten neurologischen Zustand und werden aufgrund der Antizipation postoperativer neurologischer und neurokognitiver Defizite nur selten einer Tumorresektion unterzogen.
Ziel der Arbeit. Zielsetzung dieser Arbeit war die Untersuchung eines möglichen Vorteils der Resektion von Gliomen im Corpus callosum, unter der Annahme, dass neurokognitive Defizite vor allem durch den Tumor selbst verursacht werden.
Material & Methoden. In diese prospektive Studie wurden zwischen 01/2017 und 01/2020 21 Patienten eingeschlossen, die eine Gliomresektion im Corpus callosum erfuhren. Klinische Routinedaten als auch die neurokognitiven Funktionen der Patienten wurden prä- und postoperative sowie nach 6 Monaten erhoben.
Ergebnisse. In 15 Patienten konnte eine Komplettresektion, in 6 Patienten eine subtotale Resektion mit einem Resektionsausmaß von 97,7% erzielt werden. Während der medianen Beobachtungszeit von 12,6 (2,0 – 30,9) Monaten starben 8 Patienten mit einem Glioblastom ZNS WHO Grad 4 und 1 Patient mit einem Astrozytom ZNS WHO Grad 4 nach im Median von 17 (3,8 – 20,6) Monaten, während 12 Patienten nach im Median 9,4 (2,0 – 30,9) Monaten noch am Leben waren.
Präoperativ wiesen bis zu zwei Drittel aller Patienten Beeinträchtigungen in allen kognitiven Domänen auf, trotz eines medianen Karnofsky Performance Scores von 100% (Spanne 60-100%). Postoperativ wurde in allen neurokognitiven Domänen ein zunehmender Anteil an Patienten mit Einschränkungen beobachtet. Nach 6 Monaten zeigte sich jedoch, dass signifikant weniger Patienten Beeinträchtigungen in den Domänen Aufmerksamkeit, Exekutivfunktionen, Gedächtnis und Depression aufwiesen – Funktionen, die für den Alltag der Patienten von höchster Bedeutung sind.
Fazit. Die Resultate dieser Studie unterstützen unsere Hypothese, dass für Patienten mit Gliomen, die das Corpus callosum miteinbeziehen, der Vorteil einer Tumorresektion das Risiko neuer Defizite überwiegt.
Background: With the rise of single-cell RNA sequencing new bioinformatic tools have been developed to handle specific demands, such as quantifying unique molecular identifiers and correcting cell barcodes. Here, we benchmarked several datasets with the most common alignment tools for single-cell RNA sequencing data. We evaluated differences in the whitelisting, gene quantification, overall performance, and potential variations in clustering or detection of differentially expressed genes. We compared the tools Cell Ranger version 6, STARsolo, Kallisto, Alevin, and Alevin-fry on 3 published datasets for human and mouse, sequenced with different versions of the 10X sequencing protocol.
Results: Striking differences were observed in the overall runtime of the mappers. Besides that, Kallisto and Alevin showed variances in the number of valid cells and detected genes per cell. Kallisto reported the highest number of cells; however, we observed an overrepresentation of cells with low gene content and unknown cell type. Conversely, Alevin rarely reported such low-content cells. Further variations were detected in the set of expressed genes. While STARsolo, Cell Ranger 6, Alevin-fry, and Alevin produced similar gene sets, Kallisto detected additional genes from the Vmn and Olfr gene family, which are likely mapping artefacts. We also observed differences in the mitochondrial content of the resulting cells when comparing a prefiltered annotation set to the full annotation set that includes pseudogenes and other biotypes.
Conclusion: Overall, this study provides a detailed comparison of common single-cell RNA sequencing mappers and shows their specific properties on 10X Genomics data.
Targeted delivery of nucleic acids is gaining momentum due to improved efficacy, selectivity, increased circulation time and enhanced tissue retention in target cells. Using nucleic acid-based therapies previously undruggable targets have proven now to be amenable for treatment. Currently, several methods for preparing targeted or labelled delivery vehicles for nucleic acids are based on liposomal formulations. Lipid nanoparticles (LNPs) are structurally different from liposomes and these methods should therefore be evaluated before being translated to siRNA LNPs preparation protocols. Here, we describe a robust and facile method for the preparation of targeted or fluorescently labelled siRNA LNPs. Using a copper free strain-promoted azide-alkyne cycloaddition (SPAAC) we demonstrate that post-insertion of ligand-lipid conjugates into preformed LNPs is superior to direct-surface modification because it preserves the physicochemical parameters of the LNPs. We found that the time point of solvent removal by dialysis is critical and affects the hydrodynamic diameter of the LNPs; post-insertion after dialysis shows the smallest increase in hydrodynamic diameter and polydispersity index (PDI). The post-insertion of ligand-lipid conjugates also proceeded with rapid kinetics and high efficacy over a wide temperature range. Using this optimised protocol, we generated siRNA LNPs containing both targeting and fluorescent tracking ligands allowing us to monitor siRNA LNP uptake kinetics in dependence of the targeting ligand. In aggregate, we describe a robust approach for the generation of targeted and labelled siRNA LNPs that allows their controlled and facile decoration with ligand combinations.
Background & Aims: Liver fibrosis arises from long-term chronic liver injury, accompanied by an accelerated wound healing response with interstitial accumulation of extracellular matrix (ECM). Activated hepatic stellate cells (HSC) are the main source for ECM production. MicroRNA29a (miR-29a) is a crucial antifibrotic miRNA that is repressed during fibrosis, resulting in up-regulation of collagen synthesis.
Methods; Intracellular and extracellular miRNA levels of primary and immortalized myofibroblastic HSC in response to profibrogenic stimulation by transforming growth factor β (TGFβ) or platelet-derived growth factor-BB (PDGF-BB) or upon inhibition of vesicular transport and autophagy processes were determined by quantitative polymerase chain reaction. Autophagy flux was studied by electron microscopy, flow cytometry, immunoblotting, and immunocytochemistry. Hepatic and serum miR-29a levels were quantified by using both liver tissue and serum samples from a cohort of chronic hepatitis C virus patients and a murine CCl4 induced liver fibrosis model.
Results: In our study, we show that TGFβ and PDGF-BB resulted in decrease of intracellular miR-29a and a pronounced increase of vesicular miR-29a release into the supernatant. Strikingly, miR-29a vesicular release was accompanied by enhanced autophagic activity and up-regulation of the autophagy marker protein LC3. Moreover, autophagy inhibition strongly prevented miR-29a secretion and repressed its targets’ expression such as Col1A1. Consistently, hepatic miR-29a loss and increased LC3 expression in myofibroblastic HSC were associated with increased serum miR-29a levels in CCl4-treated murine liver fibrosis and specimens of hepatitis C virus patients with chronic liver disease.
Conclusions: We provide evidence that activation-associated autophagy in HSC induces release of miR-29a, whereas inhibition of autophagy represses fibrogenic gene expression in part through attenuated miR-29a secretion.
Background_ Postural control is associated with fall risk. Patients with rheumatoid arthritis (RA) have a higher risk to fall than healthy subjects. The objective of this study was to identify associations between variables of postural control with prospective falls in patients with RA.
Methods: For the baseline, the balance performance of 289 men and women with RA, ages 24–85 years, was evaluated by SPPB, FICSIT-4 and Romberg tests. Postural sway for Romberg, semitandem, tandem and one-leg stands were measured with the Leonardo Mechanograph®. Self-reported disability was assessed using the Health Assessment Questionnaire (HAQ) and the Activity-specific Balance Confidence Scale (ABC-scale). Falls were reported in quarterly reports over a year. Univariate and multiple logistic regression analysis were used to explore any associations with falling. Receiver-operating characteristics were determined, and the area under the curve is reported.
Results`A total of 238 subjects completed the 1-year follow-up, 48 (20.2%) experienced at least one fall during the observational period. Age (OR = 1.04, CI 1.01–1.07), HAQ (OR = 1.62, 1.1–2.38), FICSIT-4 scoring 0–4 (OR = 2.38, 1.13–5.0), and one-leg standing (OR = 2.14, 1.06–4.31) showed significant associations with falls. With regard to the SPPB and ABC-scale, no statistically significant associations with falls were found. The quartiles containing the worst results of medio-lateral sway of Romberg (OR = 2.63, CI 1.03–6.69), total sway of semitandem (OR = 3.07, CI 1.10–8.57) and tandem (OR = 2.86, CI 1.06–7.69), and area of sway of semitandem (OR = 2.80, CI 1.11–7.08) stands were associated with falls.
Conclusions: The assessment of a one-leg stand seems to be a good screening tool to discriminate between high and low risk of falls in RA patients in clinical practice. A low FICSIT-4 score and several sway parameters are important predictors of falls.
Background & Aims: Elimination of chronic HBV/HDV infection remains a major global health challenge. Targeting excessive hepatitis B surface antigen (HBsAg) release may provide an interesting window of opportunity to break immune tolerance and to achieve a functional cure using additional antivirals.
Methods: We evaluated a HBsAg-specific human monoclonal antibody, as part of either a prophylactic or therapeutic strategy, against HBV/HDV infection in cell culture models and in human-liver chimeric mice. To assess prophylactic efficacy, mice were passively immunized prior to infection with HBV or HBV/HDV (coinfection and superinfection setting). Therapeutic efficacy was assessed in HBV and HBV/HDV-coinfected mice receiving 4 weeks of treatment. Viral parameters (HBV DNA, HDV RNA and HBsAg) were assessed in mouse plasma.
Results: The antibody could effectively prevent HBV/HDV infection in a dose-dependent manner with IC50 values of ∼3.5 ng/ml. Passive immunization showed complete protection of mice from both HBV and HBV/HDV coinfection. Moreover, HDV superinfection was either completely prevented or at least attenuated in HBV-infected mice. Finally, antibody treatment in mice with established HBV/HDV infection resulted in a significant decline in viremia and a concomitant drop in on-treatment HBsAg, with a moderate viral rebound following treatment cessation.
Conclusion: We present data on a valuable antibody candidate that could complement other antivirals in strategies aimed at achieving functional cure of chronic HBV and HDV infection.
Impact and implications: Patients chronically infected with HBV may eventually develop liver cancer and are at great risk of being superinfected with HDV, which worsens and accelerates disease progression. Unfortunately, current treatments can rarely eliminate both viruses from chronically infected patients. In this study, we present data on a novel antibody that is able to prevent chronic HBV/HDV infection in a mouse model with a humanized liver. Moreover, antibody treatment of HBV/HDV-infected mice strongly diminishes viral loads during therapy. This antibody is a valuable candidate for further clinical development.
Dermatologische Lehre für Medizinstudierende: ein Scoping Review publizierter Interventionsstudien

Es ist unklar, wie Medizinstudierenden Dermatologie optimal vermittelt werden sollte. Daher wurde der vorliegende Scoping Review mit folgender Fragestellung durchgeführt: „Welche dermatologischen Lehransätze für Medizinstudierende wurden in publizierten Interventionsstudien untersucht?“. Ziele waren die Identifikation und strukturierte Darstellung aller relevanten Studien. Die Methodik dieses Scoping Reviews folgte der PRISMA Extension for Scoping Reviews. Die Datenbanken Medline und Embase wurden ohne Einschränkungen bis zum 30.06.2020 durchsucht. Eingeschlossen wurden veröffentlichte Interventionsstudien, die dermatologische Lehransätze mit Medizinstudierenden untersuchten. Es erfolgten eine Kategorisierung sowie eine deskriptive Analyse der als vollständige Artikel publizierten Studien. Die Datenbanksuche ergab 36 627 Treffer. 114 Studien erfüllten alle Einschlusskriterien. Diese stammten aus 19 Ländern, wurden überwiegend seit 2010 publiziert und verteilten sich auf 64 verschiedene Journale. 32 randomisierte kontrollierte Studien wurden identifiziert. Es fanden sich verschiedenste Lehransätze, welche sowohl E-Learning als auch konventionelle Lehrformate umfassten. Die Resultate der Studien sind in strukturierten Tabellen aufgeführt. Dieser Scoping Review belegt eine hohe Zahl an weltweit publizierten Studien zur dermatologischen Lehre für Medizinstudierende. Die Vermittlung dermatologischer Inhalte scheint mit zahlreichen Lehransätzen zu gelingen, wobei Interventionen mit Berücksichtigung didaktischer Prinzipien nachweisbar erfolgreicher waren. Diese Literaturübersicht kann Lehrenden als Hilfestellung für eine evidenzbasierte Unterrichtsgestaltung dienen sowie eine Grundlage für künftige Forschungsansätze schaffen.
Results of a prospective non-interventional post-authorization safety study of idelalisib in Germany

Background: In pivotal studies, idelalisib demonstrated remarkable efficacy and manageable tolerability in patients with chronic lymphocytic leukemia (CLL) and follicular lymphoma (FL). This prospective, multicenter, non-interventional post-authorization study assessed the characteristics, clinical management, and outcome of CLL and FL patients receiving idelalisib in routine clinical practice in Germany.
Patients: Observational study in CLL and FL patients treated with idelalisib between September 2015 and December 2020.
Results: A total of 147 patients with CLL and FL were included with a median age of 75 and 71 years, respectively. More than 80% of patients presented with comorbidity and many CLL patients with documented high-risk genetic features, including del(17p)/TP53 mutation or unmutated IGHV. The median progression-free survival (PFS) and overall survival (OS) were not reached in the CLL cohort irrespective of del(17p)/TP53 or unmutated IGHV. The estimated 6-month PFS and OS rates in CLL were 82% and 92%. The estimated 6-month PFS and OS rates for FL were 32.2% and 77.2%. Overall response rates in the CLL and FL cohorts were 70.4% and 36.4%, with the presence of high-risk genetics having no negative impact. No unexpected adverse events were observed. Most frequently reported adverse drug reactions (ADRs) were diarrhea, nausea, pneumonia, rash, and fatigue.
Conclusion: This real-world study shows that idelalisib is an effective therapy for CLL and FL, regardless of age and high-risk genetic features, consistent with results from previous clinical trials. Collected safety data and the pattern of ADRs reflect those from previous studies.
The endoplasmic reticulum (ER) is the largest cellular organelle that undergoes constant turnover upon diverse functional demands and cellular signals. Removal of nonfunctional or superfluous subdomains is balanced by the parallel expansion and formation of ER membranes, leading to the dynamic exchange of ER components. In recent years, selective autophagy of the ER, termed ER-phagy, has emerged as a predominant process involved in ER degradation and maintenance of ER homeostasis. Identification of multiple ER-phagy receptors, many with additional ER-shaping functions, paved the way for our molecular understanding of ER turnover in different cells and organs. In this review, we describe the molecular principles underling the physiological functions of ER-phagy in maintaining ER homeostasis via receptor-mediated macroautophagy and elaborate current focus points of the field.
Purpose: Recent advances in the treatment algorithm of locally advanced rectal cancer (LARC) have significantly improved complete response (CR) rates and disease-free survival (DFS), but therapy resistance, with its substantial impact on outcomes and survival, remains a major challenge. Our group has recently unraveled a critical role of interleukin-1α (IL-1α) signaling in activating inflammatory cancer-associated fibroblasts (iCAFs) and mediating radiation-induced senescence, extracellular matrix (ECM) accumulation, and ultimately therapy resistance. We here summarize the recently initiated ACO/ARO/AIO-21 phase I trial, testing the IL-1 receptor antagonist (IL-1 RA) anakinra in combination with fluoropyrimidine-based chemoradiotherapy (CRT) for advanced rectal cancer.
Methods/Design: The ACO/ARO/AIO-21 is an investigator-driven, prospective, open-labeled phase I drug-repurposing trial assessing the maximum tolerated dose (MTD) of capecitabine administered concurrently to standard preoperative radiotherapy (45 Gy in 25 fractions followed by 9 Gy boost in 5 fractions) in combination with fixed doses of the IL-1RA anakinra (100 mg, days −10 to 40). Capecitabine will be administered using a 3 + 3 dose-escalation design (500 mg/m2 bid; 650 mg/m2 bid; 825 mg/m2 bid, respectively) from day 1 to day 40. Response assessment including digital rectal examination (DRE), endoscopy and pelvic magnetic resonance imaging (MRI) is scheduled 10 weeks after completion of CRT. For patients achieving clinical complete response (cCR), primary non-operative management is provided. In case of non-cCR immediate total mesorectal excision (TME) will be performed. Primary endpoint of this phase I trial is the MTD of capecitabine.
Discussion: Based on extensive preclinical research, the ACO/ARO/AIO-21 phase I trial will assess whether the IL-1RA anakinra can be safely combined with fluoropyrimidine-based CRT in rectal cancer. It will further explore the potential of IL-1 inhibition to overcome therapy resistance and improve response rates. A comprehensive translational research program will expand our understanding from a clinical perspective and may help translate the results into a randomized phase II trial.
Background: Postoperative complication rates using 3D visualization are rarely reported. The primary aim of our study is to detect a possible advantage of using 3D on postoperative complication rates in a real-world setting.
Method: With a sample size calculation for a medium effect size difference that 3D reduces significantly postoperative complications, data of 287 patients with 3D visualization and 832 with 2D procedure were screened. The groups underwent an exact propensity score-matching to be comparable. Comprehensive complication index (CCI) for every procedure was calculated and Operation Time was determined.
Results: Including 1078 patients in the study, 213 exact propensity score-matched pairs could finally be established. Concerning overall CCI (3D: 5.70 ± 13.63 vs. 2D: 3.37 ± 9.89; p = 0.076) and operation time (3D: 103.98 ± 93.26 min vs. 2D: 88.60 ±6 9.32 min; p = 0.2569) there was no significant difference between the groups.
Conclusion: Our study shows no advantage of 3D over 2D laparoscopy regarding postoperative complications in a real-world setting, the second endpoint operation time, too, was not influenced by 3D overall.

ER remodeling via ER-phagy

The endoplasmic reticulum (ER) is a hotspot for many essential cellular functions. The ER membrane is highly dynamic, which affects many cellular processes that take place within the ER. One such process is ER-phagy, a selective degradation of ER fragments (including membranes and luminal content), which serves to preserve the size of ER while adapting its morphology under basal and stress conditions. In order to be degraded, the ER undergoes selective fragmentation facilitated by specialized ER-shaping proteins that also act as ER-phagy receptors. Their ability to sense and induce membrane curvature, as well as to bridge the ER with autophagy machinery, allows for a successful ER fragmentation and delivery of these fragments to the lysosome for degradation and recycling. In this review, we provide insights into ER-phagy from the perspective of membrane remodeling. We highlight the importance of ER membrane dynamics during ER-phagy and emphasize how its dysregulation reflects on human physiology and pathology.
Combining LSD1 and JAK-STAT inhibition targets Down syndrome-associated myeloid leukemia at its core

Individuals with Down syndrome (DS) are predisposed to developing acute megakaryoblastic leukemia (ML-DS) within their first years of life [1]. Although, ML-DS is associated with a favorable prognosis, children with DS often experience severe toxicities from chemotherapy [2]. This highlights the unmet need for targeted therapies with improved risk profiles in this entity.
Consequently, the aim of this study was to investigate a novel therapeutic approach specifically tailored to intervene with hallmarks of ML-DS leukemogenesis. The evolution of ML-DS occurs in a step-wise process originating from pre-malignant transient abnormal myelopoiesis (TAM) [3]. The molecular mechanisms underlying the progression from TAM to ML-DS are not fully understood. However, it was previously shown that epigenetic changes play a pivotal role in ML-DS leukemogenesis. The lysine demethylase LSD1 was identified as a crucial player in this process, as LSD1-driven gene signatures become activated in ML-DS [4]. Accordingly, RNA-sequencing analysis of pediatric acute myeloid leukemia (AML) subtypes revealed that LSD1 was highly expressed in acute megakaryoblastic leukemia (AMKL), and especially in TAM and ML-DS patients (Supplementary Fig. 1). LSD1 is essential for hematopoiesis, particularly during granulocytic and erythroid differentiation [5], and was shown to contribute to differentiation blockade in different AML subtypes [6,7,8]. Consequently, various irreversible LSD1 inhibitors have been developed, with some currently undergoing clinical trials for AML [9]. Therefore, we sought to investigate the rational use of LSD1 inhibitors in pediatric AMKL. The non-DS-AMKL cell line M-07e and the ML-DS cell line CMK were highly sensitive to irreversible LSD1 inhibition (IC50M-07e = 9.1 nM; IC50CMK = 38.8 nM; Supplementary Fig. 2A). Testing serial dilutions of the irreversible LSD1 inhibitor in non-DS-AMKL and ML-DS patient samples expanded via xenotransplantation (see Supplementary Table 1 for patient characteristics), both entities were equally sensitive to LSD1 inhibition (non-DS-AMKL: IC50#1 = 15.0 nM, IC50#2 = 2.0 nM; ML-DS: IC50#1 = 31.2 nM, IC50#2 = 17.1 nM, IC50#3 = 3.8 nM). All dose-response curves plateaued at a certain LSD1 inhibitor concentration (Supplementary Fig. 2B). The non-linear relationship between cytotoxicity and dosage points toward proliferation arrest and differentiation in response to LSD1 inhibition. In line with this, we observed myeloid differentiation upon visual inspection (Supplementary Fig. 3A) and upregulation of the myeloid markers CD86 and CD11b after 3 days of LSD1 inhibitor treatment (Supplementary Fig. 3B).
These results revealed a potent proliferation block and induction of differentiation in non-DS-AMKL and ML-DS samples, however, the therapeutic efficacy of LSD1 inhibition may be limited by its non-linear dose-response relationship. Consequently, we aimed to design a rational drug combination to increase its anti-leukemic effects. Another hallmark of ML-DS development is the acquisition of activating mutations in Janus kinases (JAK) and cytokine receptors [4], promising potent anti-leukemic effects of the combination of LSD1 inhibition and the JAK1/JAK2 inhibitor ruxolitinib, as it was previously proposed for JAK2V617F mutated myeloproliferative neoplasms, secondary AML and a CSF3Rmut/CEBPαmut AML model [10,11,12]. Accordingly, pre-treatment with 350 nM LSD1 inhibitor for 3 days followed by exposure to serial dilutions of ruxolitinib led to synergistic growth inhibition in non-DS-AMKL and ML-DS cell lines (Supplementary Fig. 4), as well as in all ML-DS patient samples (Fig. 1A). The combination of LSD1 inhibition and ruxolitinib proved to be very effective in non-DS-AMKL blasts, however, with only additive cytotoxic effects in one of the two patient samples (Fig. 1A). Drug synergy in the ML-DS samples was confirmed when calculating the Bliss synergy scores (Fig. 1B). Interestingly, samples ML-DS #1 (JAK1mut) and #2 (wild-type for JAK1, JAK2, and JAK3, Supplementary Fig. 5) showed particularly high synergy scores (ML-DS #1 synergy score = 10.4; ML-DS #2 synergy score = 15.6; Fig. 1B). Contrary, the JAK3mut patient sample ML-DS #3 (Supplementary Fig. 5) only displayed mild drug synergy between LSD1 inhibition and ruxolitinib (synergy score = 2.0; Fig. 1B). Consequently, as ruxolitinib is a JAK1/JAK2 inhibitor, synergistic anti-leukemic effects seem to depend on JAK mutational status, which must be considered in future pre-clinical and clinical testing of this drug combination for ML-DS patients.
Purpose: To retrospectively compare the high-angled sagittal split osteotomy (HOO) and the bilateral sagittal split osteotomy (BSSO) for the correction of skeletal dysgnathias regarding intra- and postoperative complications.
Methods: The electronic medical records of all patients treated with an orthognathic surgery at the Department for Oral, Maxillofacial and Facial Plastic Surgery, University Hospital Frankfurt, Germany, between the years 2009 and 2019 were retrospectively reviewed.
Results: Two hundred ninety-one patients were included. The overall complication rates were 19.78% (BSSO) compared to 12.5% (HOO) (p = 0.14). Significant differences were found regarding the operation time (HOO < BSSO, p = 0.02), material failure (HOO > BSSO, p = 0.04), and early recurrence requiring revision surgery (HOO < BSSO, p = 0.002). The use of a ramus plate significantly reduced the risk of plate failure (2.8% < 13.6%, p = 0.05). More bad splits (p = 0.08) and early sensory disorders (p = 0.07) occurred in the BSSO group.
Conclusion: The HOO presents a possible alternative to the BSSO since newly developed osteosynthesis material significantly reduces the risk of material failure. The BSSO is accompanied by higher risks of developing complications like a bad split and sensory disorders but, however, remains the standard for large anterior–posterior transpositions of the mandible.
Hintergrund und Fragestellung: Die Severe acute respiratory syndrome coronavirus type 2(SARS-CoV-2)-Pandemie hat die Ausbildung von Medizinstudierenden grundlegend verändert. Die Notwendigkeit von Kontaktbeschränkungen und die damit einhergehende Forderung nach Distanzunterricht hat dazu geführt, dass innerhalb kurzer Zeit digitale Lehrformate umgesetzt werden mussten. Ziel dieser Arbeit war die Auswertung der studentischen Evaluationsergebnisse für virtuellen Unterricht im Fach Hals-Nasen-Ohren-Heilkunde während der SARS-CoV-2-Pandemie und ein Vergleich mit den zuvor erhobenen Evaluationsergebnissen unter Präsenzbedingungen.
Material und Methoden: Untersucht wurden die Evaluationsergebnisse für die Blockpraktika im Wintersemester 2020/21 und im Sommersemester 2021, die in einem virtuellen Format mit kurzer Präsenzphase durchgeführt wurden, sowie die der komplett im konventionellen Präsenzformat durchgeführten Praktika von Sommersemester 2018 bis Wintersemester 2019/20. Die anonyme Befragung der Studierenden bezog sich auf verschiedene Aspekte der Lehrveranstaltung, wie z. B. Organisation, Didaktik und Lernatmosphäre.
Ergebnisse: Von 16 abgefragten Kategorien zeigten 14 (87,5%) signifikant bessere Evaluationsergebnisse für die virtuellen Praktika verglichen mit den zuvor im Präsenzformat durchgeführten Praktika. Diese sehr positive Bewertung des digitalen Lehrangebots zeigte im Pandemieverlauf über die Dauer von zwei Semestern keine signifikante Änderung.
Schlussfolgerung: Die vorliegenden Daten belegen die hohe Akzeptanz eines digitalen Lehrangebots im Fach HNO-Heilkunde für Studierende. Auch wenn unerlässliche Bestandteile der ärztlichen Ausbildung, wie der Unterricht am Patienten und das Erlernen klinisch-praktischer Fertigkeiten, weiterhin nur im Präsenzformat realisiert werden können, legen die Ergebnisse nahe, dass digitale Elemente auch nach der SARS-CoV-2-Pandemie eine Rolle im Medizinstudium spielen könnten.
Acute myeloid leukemia (AML) is a neoplastic disease of an early myeloid precursor cell in hematopoiesis. It leads to the accumulation of monoclonal cells in the bone marrow and the peripheral blood, showing a differentiation block and deregulated self-renewal. Frequently, the leukemic cells exhibit genetic aberrations with reciprocal chromosomal translocations. These translocations induce the formation of a fusion protein, that can lead to new cellular functions and a transformation into a leukemic cell. Common chromosomal translocation in AML are t(8;21) or t(15;17), which cause the formation of the fusion proteins AML1/ETO and PML/RARα and determine the leukemic phenotype of the AML.
The translocation t(6;9) leads to the formation of the fusion protein DEK/CAN and is of special interest, because of its association with mostly young patients and a very aggressive course of the disease. The fusion product induces leukemia in a small subset of hematopoietic stem cells, but its mechanism of leukemogenesis is greatly unknown.
The intention of this work was to characterize the DEK/CAN-induced AML on a molecular genetic level to gain a deeper understanding of the disease pathogenesis. Therefore, gene expression analysis with polymerase chain reaction (PCR) and microarray analysis was performed.
To detect DEK/CAN in different cell lines by PCR and real-time quantitative PCR (qPCR), specific primers and probes were designed, and a standardized workflow was established. Emphasis was placed on the optimization of RNA isolation, DNase treatment, cDNA synthesis with following PCR and qPCR, which enabled the detection of the fusion product DEK/CAN in the cell lines 32B, Phoenix and FKH-1. To quantify the fusion product DEK/CAN, the method of qPCR with absolute and relative quantification was used. Absolute quantification enabled the calculation of an exact copy number of the fusion transcript DEK/CAN with a detection limit of 50 copies/µl at a sensitivity of 10-6, which is of importance in determining the minimal residual disease (MRD) of patients with DEK/CAN-positive AML. MRD detection by qPCR is a highly sensitive diagnostic method to identify leukemic cells, even in low cell counts. This enables a thorough evaluation of the treatment response and allows an early detection of changes in the MRD level as part of the remission control.
Additionally, a microarray gene expression analysis was performed to identify alterations in relevant target genes and associated signaling pathways in DEK/CAN-positive cells.
Because of DEK/CAN’s potential to induce leukemia in a subset of hematopoietic stem cells, Sca+/Lin- cells of the bone marrow of C57Bl/6 mice were used and transfected with the gene products DEK/CAN and PML/RARα. Microarray analysis led to the identification of 16 different genes of interest, which demonstrated significant alterations of gene expression in DEK/CAN-positive cells. They were validated and quantified with TaqMan assay assisted qPCR. The elevated expression of the transcription factors TRIM25, HIF1α and ATF2, in DEK/CAN-positive cells, indicated an altered transcription factor activity and interaction with DNA in the nucleus. The localization of DEK/CAN in the nucleus emphasizes this assumption. Also, the upregulated expression of the nuclear export receptor XPO1 suggested changes in nuclear transport processes and impaired export activity in DEK/CAN-positive cells.
Furthermore, the results demonstrated changes of gene expression in genes that are involved in the JAK/STAT signaling pathway. PTPRC, the Protein Tyrosine Phosphatase Receptor Type C, functions as a direct inhibitor of JAKs (Janus Kinases) and STATs (Signal Transducers and Activators of Transcription) and their associated signaling pathway.
It was shown that the gene expression of PTPRC was significantly reduced in DEK/CAN-positive cells. This allowed the assumption, that the reduced expression of PTPRC led to a loss of inhibition and thus a consecutive hyperactivation of the JAK/STAT signaling pathway. This hypothesis was supported by an independent activation of PIM1, a target gene of STAT5 and the activation of LMO2, a direct target gene of JAK2. In addition, the transmembrane receptor CSF1R, which is directly involved in STAT activation, also showed an upregulation in gene expression.
The results of this work show an activation of the JAK/STAT signaling pathway in DEK/CAN-positive cells, which may be a key mechanism in DEK/CAN-induced leukemogenesis.
Considering treatment options in the future, the addition of targeted therapy, such as pan-JAK inhibitors, to the standard therapy, could be a chance to improve the overall survival rate and the prognosis of t(6;9)-positive AML.
Das Capillary-Leak-Syndrom (CLS) präsentiert sich als plötzlicher Verlust von intravasaler Flüssigkeit ins Interstitium und kann als Komplikation nach einer Stammzelltransplantation (SZT) auftreten. Das Ziel der vorliegenden Studie war es, die CLS-Inzidenz, Risikofaktoren für das Auftreten und den Einfluss auf das Überleben nach SZT in einer pädiatrischen Kohorte allogener SZT-Empfänger zu bestimmen. Wir untersuchten die klinischen Berichte aller Patienten unter 18 Jahren, die in der Abteilung für Stammzelltransplantation der Klinik für Kinder- und Jugendmedizin des Universitätsklinikums Frankfurt am Main zwischen Januar 2002 und Mai 2012 allogen stammzelltransplantiert wurden. 234 Patienten erhielten 275 SZT im oben genannten Zeitraum. In 15 Fällen (5,5 %) entwickelten Patienten ein CLS.
Bedingung für die Vergabe der CLS-Diagnose war das Vorhandensein dieser drei Kriterien: Gewichtszunahme von über 3 % des Körpergewichts in 24 Stunden, positive Ein-/Ausfuhrbilanz trotz Diuretikagabe und Ödeme.
Die Wahrscheinlichkeit, ein CLS zu entwickeln war signifikant höher bei Patienten, die auch unter einer Sepsis litten (p < 0,001). Patienten mit CLS hatten zudem ein höheres Risiko innerhalb der ersten 30 Tage nach SZT an einer GvHD zu erkranken (p = 0,005).
10 der 15 CLS-Patienten mussten intensivmedizinisch behandelt werden.
CLS hängt signifikant mit dem Gesamtüberleben an Tag + 100 und Tag + 365 nach SZT zusammen (p < 0,001), indem es einen starken prädiktiven Faktor für die TRM an Tag + 100 und Tag + 365 nach SZT darstellt (p < 0,001).
CLS ist eine schwere Komplikation bei pädiatrischen SZT-Empfängern. Der biologische Zusammenhang zwischen Sepsis, GvHD und CLS-Entwicklung im Hinblick auf Zytokinfreisetzung und Endothelschaden sollte in weiteren Studien untersucht werden, um neue zielgerichtete Therapien zu unterstützen.
Legionella pneumophila, a Gram-negative intracellular bacterium, is one of the major causes of Legionnaires’ disease, a specific type of atypical pneumonia. Despite intensive research efforts that elucidated many relevant structural, molecular and medical insights into Legionella’s pathogenicity, Legionnaires’ disease continues to present an ongoing public health concern. Legionella’s virulence is based on its ability to simultaneously hijack multiple molecular pathways of the host cell to ensure its fast replication and dissemination. Legionella usurps the host ubiquitin system through multiple effector proteins, using the advantage of both conventional and unconventional (phosphoribosyl-linked) ubiquitination, thus providing optimal conditions for its replication. In this review, we summarize the current understanding of L. pneumophila from medical, biochemical and molecular perspectives. We describe the clinical disease presentation, its diagnostics and treatment, as well as host-pathogen interactions, with the emphasis on the ability of Legionella to target the host ubiquitin system upon infection. Furthermore, the interdisciplinary use of innovative technologies enables better insights into the pathogenesis of Legionnaires’ disease and provides new opportunities for its treatment and prevention.
Recent scientific evidence suggests that chronic pain phenotypes are reflected in metabolomic changes. However, problems associated with chronic pain, such as sleep disorders or obesity, may complicate the metabolome pattern. Such a complex phenotype was investigated to identify common metabolomics markers at the interface of persistent pain, sleep, and obesity in 71 men and 122 women undergoing tertiary pain care. They were examined for patterns in d = 97 metabolomic markers that segregated patients with a relatively benign pain phenotype (low and little bothersome pain) from those with more severe clinical symptoms (high pain intensity, more bothersome pain, and co-occurring problems such as sleep disturbance). Two independent lines of data analysis were pursued. First, a data-driven supervised machine learning-based approach was used to identify the most informative metabolic markers for complex phenotype assignment. This pointed primarily at adenosine monophosphate (AMP), asparagine, deoxycytidine, glucuronic acid, and propionylcarnitine, and secondarily at cysteine and nicotinamide adenine dinucleotide (NAD) as informative for assigning patients to clinical pain phenotypes. After this, a hypothesis-driven analysis of metabolic pathways was performed, including sleep and obesity. In both the first and second line of analysis, three metabolic markers (NAD, AMP, and cysteine) were found to be relevant, including metabolic pathway analysis in obesity, associated with changes in amino acid metabolism, and sleep problems, associated with downregulated methionine metabolism. Taken together, present findings provide evidence that metabolomic changes associated with co-occurring problems may play a role in the development of severe pain. Co-occurring problems may influence each other at the metabolomic level. Because the methionine and glutathione metabolic pathways are physiologically linked, sleep problems appear to be associated with the first metabolic pathway, whereas obesity may be associated with the second.
Objective: This study was undertaken to calculate epilepsy-related direct, indirect, and total costs in adult patients with active epilepsy (ongoing unprovoked seizures) in Germany and to analyze cost components and dynamics compared to previous studies from 2003, 2008, and 2013. This analysis was part of the Epi2020 study.
Methods: Direct and indirect costs related to epilepsy were calculated with a multicenter survey using an established and validated questionnaire with a bottom-up design and human capital approach over a 3-month period in late 2020. Epilepsy-specific costs in the German health care sector from 2003, 2008, and 2013 were corrected for inflation to allow for a valid comparison.
Results: Data on the disease-specific costs for 253 patients in 2020 were analyzed. The mean total costs were calculated at €5551 (±€5805, median = €2611, range = €274–€21 667) per 3 months, comprising mean direct costs of €1861 (±€1905, median = €1276, range = €327–€13 158) and mean indirect costs of €3690 (±€5298, median = €0, range = €0–€11 925). The main direct cost components were hospitalization (42.4%), antiseizure medication (42.2%), and outpatient care (6.2%). Productivity losses due to early retirement (53.6%), part-time work or unemployment (30.8%), and seizure-related off-days (15.6%) were the main reasons for indirect costs. However, compared to 2013, there was no significant increase of direct costs (−10.0%), and indirect costs significantly increased (p < .028, +35.1%), resulting in a significant increase in total epilepsy-related costs (p < .047, +20.2%). Compared to the 2013 study population, a significant increase of cost of illness could be observed (p = .047).
Significance: The present study shows that disease-related costs in adult patients with active epilepsy increased from 2013 to 2020. As direct costs have remained constant, this increase is attributable to an increase in indirect costs. These findings highlight the impact of productivity loss caused by early retirement, unemployment, working time reduction, and seizure-related days off.
In gastric cancer (GC), there are four molecular subclasses that indicate whether patients respond to chemotherapy or immunotherapy, according to the TCGA. In clinical practice, however, not every patient undergoes molecular testing. Many laboratories have used well-implemented in situ techniques (IHC and EBER-ISH) to determine the subclasses in their cohorts. Although multiple stains are used, we show that a staining approach is unable to correctly discriminate all subclasses. As an alternative, we trained an ensemble convolutional neuronal network using bagging that can predict the molecular subclass directly from hematoxylin–eosin histology. We also identified patients with predicted intra-tumoral heterogeneity or with features from multiple subclasses, which challenges the postulated TCGA-based decision tree for GC subtyping. In the future, deep learning may enable targeted testing for molecular subtypes and targeted therapy for a broader group of GC patients. © 2022 The Authors. Feature selection is a common step in data preprocessing that precedes machine learning to reduce data space and the computational cost of processing or obtaining the data. Filtering out uninformative variables is also important for knowledge discovery. By reducing the data space to only those components that are informative to the class structure, feature selection can simplify models so that they can be more easily interpreted by researchers in the field, reminiscent of explainable artificial intelligence. Knowledge discovery in complex data thus benefits from feature selection that aims to understand feature sets in the thematic context from which the data set originates. However, a single variable selected from a very small number of variables that are technically sufficient for AI training may make little immediate thematic sense, whereas the additional consideration of a variable discarded during feature selection could make scientific discovery very explicit. In this report, we propose an approach to explainable feature selection (XFS) based on a systematic reconsideration of unselected features. The difference between the respective classifications when training the algorithms with the selected features or with the unselected features provides a valid estimate of whether the relevant features in a data set have been selected and uninformative or trivial information was filtered out. It is shown that revisiting originally unselected variables in multivariate data sets allows for the detection of pathologies and errors in the feature selection that occasionally resulted in the failure to identify the most appropriate variables.
Several lines of evidence suggest the ligand-sensing transcription factor Nurr1 as a promising target to treat neurodegenerative diseases. Nurr1 modulators to validate and exploit this therapeutic potential are rare, however. To identify novel Nurr1 agonist chemotypes, we have employed the Nurr1 activator amodiaquine as template for microscale analogue library synthesis. The first set of analogues was based on the 7-chloroquiolin-4-amine core fragment of amodiaquine and revealed superior N-substituents compared to diethylaminomethylphenol contained in the template. A second library of analogues was subsequently prepared to replace the chloroquinolineamine scaffold. The two sets of analogues enabled a full scaffold hop from amodiaquine to a novel Nurr1 agonist sharing no structural features with the lead but comprising superior potency on Nurr1. Additionally, pharmacophore modeling based on the entire set of active and inactive analogues suggested key features for Nurr1 agonists.
1. Locating an optimal oviposition site can be a challenging task for female insects, especially when dealing with a patchy, unpredictable and ephemeral food source such as carrion. Understanding the biotic and abiotic parameters that influence the oviposition behaviour of necrophagous flies is not just of great biological importance but also essential for their application in legal investigations.
2. In this study, we monitored the oviposition activity of necrophagous flies (Calliphoridae, Sarcophagidae) using mouse carcasses in an urban (city) and a rural (mixed forest) habitat in Frankfurt/Germany over a 2-year period.
3. Over 240 sampling days, 220,963 larvae of 4 blow fly species and 1 flesh fly were sampled. The most abundant species was the blow fly Lucilia ampullacea, followed by its family members Calliphora vicina and Lucilia caesar, the flesh fly Sarcophaga caerulescens and Lucilia sericata. Up to seven environmental parameters were statistically significant predictors for a colonisation of the carcasses, leading to unique patterns of seasonal and daily oviposition activity for all five species.
4. Overall, the analysis showed that the seasonal adaption (the phenology of each species), the habitat (rural vs. urban) as well as temperature are the most important factors influencing the oviposition behaviour and activity of necrophagous blow flies and flesh flies.
Gene therapy (GT) is becoming a realistic treatment option for patients with haemophilia. Outside clinical trials, the complexity and potential complications of GT will pose unprecedented challenges to haemophilia care centres.AIM: To explore the potential use of electronic tools to improve the delivery of GT under real-world conditions.METHODS: Considering the hub-and-spoke model, the GTH working group on GT considered the entire patient pathway and reached consensus on requirements for an integrative software tool to secure documenting and sharing information between treaters, pharmacies and patients.RESULTS: Six steps of the gene therapy process were identified, each requiring completion of the previous step as a prerequisite for entry. The responsibilities of GT dosing and follow-up treatment centres, read/write access rules, and the minimum data set were outlined. Data contributed by patients through mobile devices was also considered.CONCLUSION: Important information needs to be shared between patients and treatment centres in a real-world GT hub-and-spoke model. Collecting and sharing this information in well-organised electronic applications will not only improve patient care but also enable national and international data collection in clinical registries...
This survey reports on the DNA identification and occurrence of Culex torrentium and Cx. pipiens s.s. in Belgium. These native disease-vector mosquito species are morphologically difficult to separate, and the biotypes of Cx. pipiens s.s. are morphologically indistinguishable. Culex torrentium and Cx. pipiens s.s. were identified using the COI and ACE2 loci. We recorded 1248 Cx. pipiens s.s. and 401 Cx. torrentium specimens from 24 locations in Belgium (collected between 2017 and 2019). Culex pipiens biotypes pipiens and molestus, and their hybrids, were differentiated using fragment-size analysis of the CQ11 locus (956 pipiens and 227 molestus biotype specimens, 29 hybrids). Hybrids were observed at 13 out of 16 sympatric sites. These results confirm that both species are widespread in Belgium, but while Cx. torrentium revealed many COI haplotypes, Cx. pipiens s.s. showed only one abundant haplotype. This latter observation may either reflect a recent population-wide demographic or range expansion, or a recent bottleneck, possibly linked to a Wolbachia infection. Finally, new evidence is provided for the asymmetric but limited introgression of the molestus biotype into the pipiens biotype.
Objective: This study was undertaken to quantify epilepsy-related costs of illness (COI) in Germany and identify cost-driving factors.
Methods: COI were calculated among adults with epilepsy of different etiologies and severities. Multiple regression analysis was applied to determine any epilepsy-related and sociodemographic factors that serve as cost-driving factors.
Results: In total, 486 patients were included, with a mean age of 40.5 ± 15.5 years (range = 18–83 years, 58.2% women). Mean 3-month COI were estimated at €4911, €2782, and €2598 for focal, genetic generalized, and unclassified epilepsy, respectively. The mean COI for patients with drug-refractory epilepsy (DRE; €7850) were higher than those for patients with non-DRE (€4720), patients with occasional seizures (€3596), or patients with seizures in remission for >1 year (€2409). Identified cost-driving factors for total COI included relevant disability (unstandardized regression coefficient b = €2218), poorer education (b = €2114), living alone (b = €2612), DRE (b = €1831), and frequent seizures (b = €2385). Younger age groups of 18–24 years (b = −€2945) and 25–34 years (b = −€1418) were found to have lower overall expenditures. A relevant disability (b = €441), DRE (b = €1253), frequent seizures (b = €735), and the need for specialized daycare (b = €749) were associated with higher direct COI, and poorer education (b = €1969), living alone (b = €2612), the presence of a relevant disability (b = €1809), DRE (b = €1831), and frequent seizures (b = €2385) were associated with higher indirect COI.
Significance: This analysis provides up-to-date COI data for use in further health economics analyses, highlighting the high economic impacts associated with disease severity, disability, and disease-related loss of productivity among adult patients with epilepsy. The identified cost drivers could be used as therapeutic and socioeconomic targets for future cost-containment strategies.
BH3 mimetics are promising novel anticancer therapeutics. By selectively inhibiting BCL-2, BCL-xL, or MCL-1 (i.e. ABT-199, A-1331852, S63845) they shift the balance of pro- and anti-apoptotic proteins in favor of apoptosis. As Bromodomain and Extra Terminal (BET) protein inhibitors promote pro-apoptotic rebalancing, we evaluated the potential of the BET inhibitor JQ1 in combination with ABT-199, A-1331852 or S63845 in rhabdomyosarcoma (RMS) cells. The strongest synergistic interaction was identified for JQ1/A-1331852 and JQ1/S63845 co-treatment, which reduced cell viability and long-term clonogenic survival. Mechanistic studies revealed that JQ1 upregulated BIM and NOXA accompanied by downregulation of BCL-xL, promoting pro-apoptotic rebalancing of BCL-2 proteins. JQ1/A-1331852 and JQ1/S63845 co-treatment enhanced this pro-apoptotic rebalancing and triggered BAK- and BAX-dependent apoptosis since a) genetic silencing of BIM, BAK or BAX, b) inhibition of caspase activity with zVAD.fmk and c) overexpression of BCL-2 all rescued JQ1/A-1331852- and JQ1/S63845-induced cell death. Interestingly, NOXA played a different role in both treatments, as genetic silencing of NOXA significantly rescued from JQ1/A-1331852-mediated apoptosis but not from JQ1/S63845-mediated apoptosis. In summary, JQ1/A-1331852 and JQ1/S63845 co-treatment represent new promising therapeutic strategies to synergistically trigger mitochondrial apoptosis in RMS.
Objective: This paper presents a novel digital workflow that expedites and facilitates the manufacturing of high-end full-ceramic restorations based on “Print and Press”-Technology combined with 3D-printed colored 3D-models.
Clinical considerations: Despite ongoing innovations and developments in the digital workflow, the precision, and the final esthetic outcome is still limited compared with conventional press ceramics. The proposed method combines the advantages of digital scan- and design technologies with the proven conventional press-technology to accomplish high-end full-ceramic restorations. The restoration is digitally designed, the data set is 3D-printed in resin that can be burned out, subsequently conventionally embedded and pressed. Final esthetic finishing of the partial restorations is done on a 3D-printed physical colored 3D-model.
Conclusion: The report describes synergetic effects of digital and analog procedures. 3D-printed colored 3D-models can positively support the manufacturing of full ceramic restorations regarding their optical integration. Therefore, the use of 3D-printed colored 3D-models signifies a new innovative technique with many promising application areas.
Clinical significance: The combination of excellent clinical long-term data for pressed ceramic restorations and proven digital processes, like intraoral scanning, design, and additive manufacturing, in the dental field promise an individual workflow for predictability and excellent esthetics.
Propranolol as a potentially novel treatment of arteriovenous malformations: from bench to bedside

Background: Propranolol is a non-selective blocker of the β-adrenergic receptor and has been used for treatment of proliferative infantile hemangiomas. The vasoconstrictive and antiangiogenic effects of propranolol led us to explore its potential application for the treatment of AVMs.
Methods: AVM tissue was cultured after surgical resection in the presence of 100μM propranolol or solvent DMSO. After incubation for 72 hours, tissue was harvested for testing. The expression levels of SDF1α, CXCR4, VEGF and HIF-1 was measured by rt-PCR. Furthermore, data of patients in 2 vascular centres harboring AVM was retrospectively interrogated for a time period of 20 years. The database included information about hemorrhage, AVM size and antihypertensive medication. Descriptive analyses were performed, focusing on the risk of hemorrhage, size of the lesion at presentation and clinical follow-up in patients on β-blocker medication versus those who were not.
Results: Among 483 patients, 73 (15%) were under β-blocker-treatment. 48% AVMs presented with hemorrhage at diagnosis. Patients under β-blocker-treatment had a lower risk of hemorrhage at the time of diagnosis in a univariate analysis (p<0,0001;OR13). Patients under β-blocker-treatment showed a significant higher chance for a lower Spetzler-Martin-grade ≤III (p<0,0001;OR6,5) and a lower risk for the presence of an associated aneurysm (p<0,0001;OR3,6).
Multivariate analysis including Spetzler-Martin-Grading, young age ≤50, presence of associated aneurysm and β-blocker-treatment showed reduced risk for hemorrhage under β-blocker-treatment (p<0,01,OR0,2).
The expression of CXCR4 was suppressed by propranolol most likely through the HIF-1-pathways. The gene-expression of vasculogenesis factors was decreased in with propranolol incubated AVMs.
Conclusion: β-Blocker medication seems to be associated with a decreased risk of AVM-related hemorrhage and AVM-size at presentation or during follow-up. Propranolol inhibits SDF1α-induced vasculogenesis by suppressing the expression of CXCR4 most likely through the HIF-1-pathways. Therefore, SDF1α/CXCR4 axis plays an important role in the vasculogenesis and migration of inflammatory cells in AVM lesions.
Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease of the hair follicles leading to painful lesions, associated with increased levels of pro-inflammatory cytokines. Numerous guidelines recommend antibiotics like clindamycin and rifampicin in combination, as first-line systemic therapy in moderate-to-severe forms of inflammation. HS has been proposed to be mainly an auto-inflammatory disease associated with but not initially provoked by bacteria. Therefore, it has to be assumed that the pro-inflammatory milieu previously observed in HS skin is not solely dampened by the bacteriostatic inhibition of DNA-dependent RNA polymerase. To further clarify the mechanism of anti-inflammatory effects of rifampicin, ex vivo explants of lesional HS from 8 HS patients were treated with rifampicin, and its effect on cytokine production, immune cells as well as the expression of Toll-like receptor 2 (TLR2) were investigated. Analysis of cell culture medium of rifampicin-treated HS explants revealed an anti-inflammatory effect of rifampicin that significantly inhibiting interleukin (IL)-1β, IL-6, IL-8, IL-10 and tumour necrosis factor (TNF)-α production. Immunohistochemistry of the rifampicin-treated explants suggested a tendency for it to reduce the expression of TLR2 while not affecting the number of immune cells.
Background and Aim: The main disadvantage of plastic stents is the high rate of stent occlusion. The usual replacement interval of biliary plastic stents is 3 months. This study aimed to investigate if a shorter interval of 6–8 weeks impacts the median premature exchange rate (mPER) in benign and malignant biliary strictures.
Methods: All cases with endoscopic retrograde cholangiopancreatography (ERCP) and plastic stent placement were retrospectively analyzed since establishing an elective replacement interval of every 6–8 weeks at our institution and mPER was determined.
Results: A total of 3979 ERCPs (1199 patients) were analyzed, including 1262 (31.7%) malignant and 2717 (68.3%) benign cases, respectively. The median stent patency (mSP) was 41 days (range 14–120) for scheduled stent exchanges, whereas it was 17 days (1–75) for prematurely exchanged stents. The mPER was significantly higher for malignant (28.1%, 35–50%) compared with benign strictures (15.2%, 10–28%), P < 0.0001, respectively. mSP was significantly shorter in cases with only one stent (34 days [1–87] vs 41 days [1–120]) and in cases with only a 7-Fr stent (28 days [2–79]) compared with a larger stent (34 days [1–87], P = 0.001). Correspondingly, mPER was significantly higher in cases with only one stent (23% vs 16.2%, P < 0.0001) and only a 7-Fr stent (31.3% vs 22.4%, P = 0.03).
Conclusion: A shorter replacement interval does not seem to lead to a clinically meaningful reduction of mPER in benign and malignant strictures. Large stents and multiple stenting should be favored as possible.
Background and purpose: Transient splenial oedema, also known as reversible splenial lesion syndrome (RESLES), is a rare magnetic resonance imaging (MRI) finding that presents as a round or ovoid focal oedema in the posterior corpus callosum, and is associated with a wide range of clinical conditions. The aetiology of RESLES is not fully clear. We aimed to investigate conflicting pathophysiological hypotheses by measuring local glucose metabolism in patients with RESLES.
Methods: We retrospectively analysed patients with RESLES after reductions in antiseizure medications during in-hospital video electroencephalography monitoring. We measured local glucose uptake using positron emission tomography/computed tomography and compared matched cohorts of patients with and without MRI evidence of RESLES using nonparametric tests.
Results: Local glucose metabolism in the splenium of seven patients with RESLES was not significantly different from the glucose metabolism of the seven patients in the matched cohort. This was true using both regular and normalized standardized glucose uptake value calculation methods (p = 0.902 and p = 0.535, respectively).
Conclusion: We found no evidence of local glucose hypometabolism in RESLES, which supports previous pathophysiological considerations that suggest that RESLES is an intercellular, intramyelinic oedema rather than a typical intracellular cytotoxic oedema, which is not reversible.
