Background: Tisagenlecleucel, an anti-CD19 chimeric antigen receptor T cell therapy, has demonstrated efficacy in children and young adults with relapsed/refractory B cell acute lymphoblastic leukemia (B-ALL) in two multicenter phase 2 trials (ClinicalTrials.gov, NCT02435849 (ELIANA) and NCT02228096 (ENSIGN)), leading to commercialization of tisagenlecleucel for the treatment of patients up to age 25 years with B-ALL that is refractory or in second or greater relapse.
Methods: A pooled analysis of 137 patients from these trials (ELIANA: n=79; ENSIGN: n=58) was performed to provide a comprehensive safety profile for tisagenlecleucel.
Results: Grade 3/4 tisagenlecleucel-related adverse events (AEs) were reported in 77% of patients. Specific AEs of interest that occurred ≤8 weeks postinfusion included cytokine-release syndrome (CRS; 79% (grade 4: 22%)), infections (42%; grade 3/4: 19%), prolonged (not resolved by day 28) cytopenias (40%; grade 3/4: 34%), neurologic events (36%; grade 3: 10%; no grade 4 events), and tumor lysis syndrome (4%; all grade 3). Treatment for CRS included tocilizumab (40%) and corticosteroids (23%). The frequency of neurologic events increased with CRS severity (p<0.001). Median time to resolution of grade 3/4 cytopenias to grade ≤2 was 2.0 (95% CI 1.87 to 2.23) months for neutropenia, 2.4 (95% CI 1.97 to 3.68) months for lymphopenia, 2.0 (95% CI 1.87 to 2.27) months for leukopenia, 1.9 (95% CI 1.74 to 2.10) months for thrombocytopenia, and 1.0 (95% CI 0.95 to 1.87) month for anemia. All patients who achieved complete remission (CR)/CR with incomplete hematologic recovery experienced B cell aplasia; however, as nearly all responders also received immunoglobulin replacement, few grade 3/4 infections occurred >1 year postinfusion.
Conclusions: This pooled analysis provides a detailed safety profile for tisagenlecleucel during the course of clinical trials, and AE management guidance, with a longer follow-up duration compared with previous reports.
Treatment of chronic myeloid leukemia (CML) and Philadelphia chromosome-positive acute leukemia (Ph+ ALL) has been revolutionized with the advent of tyrosine kinase inhibitors (TKIs). Most patients with CML achieve long-term survival similar to individuals without CML due to treatment with TKIs not only in frontline but also in further lines of therapy. The third-generation TKI ponatinib has demonstrated efficacy in patients with refractory CML and Ph+ ALL. Ponatinib is currently the most potent TKI in this setting demonstrating activity against T315I mutant clones. However, ponatinib’s safety data revealed a dose-dependent, increased risk of serious cardiovascular (CV) events. Guidance is needed to evaluate the benefit–risk profile of TKIs, such as ponatinib, and safety measures to prevent treatment-associated CV events. An expert panel of German hematologists and cardiologists summarize current evidence regarding ponatinib’s efficacy and CV safety profile. We propose CV management strategies for patients who are candidates for ponatinib.
Background: Reactive oxygen species (ROS) and reactive nitrogen species (RNS) are produced during hemorrhagic shock and resuscitation (H/R), which may contribute to multiple organ failure. The AIM of this study was to test the hypothesis that green tea (Camellia sinenesis) extract containing 85% polyphenols decreases injury after H/R in rats by scavenging ROS and RNS. Method: S: Female Sprague Dawley rats were given 100 mg polyphenol extract/kg body weight or vehicle 2 h prior to hemorrhagic shock. H/R was induced by two protocols: 1) withdrawal of blood to a mean arterial pressure of 40 mm Hg followed by further withdrawals to decrease blood pressure progressively to 28 mm Hg over 1 h (severe), and 2) withdrawal of blood to a sustained hypotension of 40 mm Hg for 1 h (moderate). Rats were then resuscitated over 1 h with 60% of the shed blood volume plus twice the shed blood volume of lactated Ringer's solution. Serum samples were collected at 10 min and 2 h after resuscitation. At 2 or 18 h, livers were harvested for cytokine and 3-nitrotyrosine quantification, immunohistochemical detection of 4-hydroxynonenol (4-HNE) and inducible nitric oxide synthase (iNOS) protein expression. Results: After severe H/R, 18-h survival increased from 20% after vehicle to 70% after polyphenols (p<0.05). After moderate H/R, survival was greater (80%) and not different between vehicle and polyphenols. In moderate H/R, serum alanine aminotransferase (ALT) increased at 10 min and 2 h postresuscitation to 345 and 545 IU/L, respectively. Polyphenol treatment blunted this increase to 153 and 252 IU/L at 10 min and 2 h (p<0.01). Polyphenols also blunted increases in liver homogenates of TNFalpha (7.0 pg/mg with vehicle vs. 4.9 pg/mg with polyphenols, p<0.05), IL-1beta (0.80 vs. 0.37 pg/mg, p<0.05), IL-6 (6.9 vs. 5.1 pg/mg, p<0.05) and nitrotyrosine (1.9 pg/mg vs. 0.6 pg/mg, p<0.05) measured 18 h after H/R. Hepatic 4-HNE immunostaining indicative of lipid peroxidation also decreased from 4.8% after vehicle to 1.5% after polyphenols (p<0.05). By contrast, polyphenols did not block increased iNOS expression at 2 h after H/R. CONCLUSION: Polyphenols decrease ROS/RNS formation and are beneficial after hemorrhagic shock and resuscitation.
Background: Autism spectrum disorders (ASDs) are a group of highly heritable neurodevelopmental disorders which are characteristically comprised of impairments in social interaction, communication and restricted interests/behaviours. Several cell adhesion transmembrane leucine-rich repeat (LRR) proteins are highly expressed in the nervous system and are thought to be key regulators of its development. Here we present an association study analysing the roles of four promising candidate genes - LRRTM1 (2p), LRRTM3 (10q), LRRN1 (3p) and LRRN3 (7q) - in order to identify common genetic risk factors underlying ASDs.
Methods: In order to gain a better understanding of how the genetic variation within these four gene regions may influence susceptibility to ASDs, a family-based association study was undertaken in 661 families of European ancestry selected from four different ASD cohorts. In addition, a case-control study was undertaken across the four LRR genes, using logistic regression in probands with ASD of each population against 295 ECACC controls.
Results: Significant results were found for LRRN3 and LRRTM3 (P < 0.005), using both single locus and haplotype approaches. These results were further supported by a case-control analysis, which also highlighted additional SNPs in LRRTM3.
Conclusions: Overall, our findings implicate the neuronal leucine-rich genes LRRN3 and LRRTM3 in ASD susceptibility.
Borrelia burgdorferi sensu lato (Bbsl), the causative agent of Lyme disease, establishes an initial infection in the host’s skin following a tick bite, and then disseminates to distant organs, leading to multisystem manifestations. Tick-to-vertebrate host transmission requires that Bbsl survives during blood feeding. Complement is an important innate host defense in blood and interstitial fluid. Bbsl produces a polymorphic surface protein, CspA, that binds to a complement regulator, Factor H (FH) to block complement activation in vitro. However, the role that CspA plays in the Bbsl enzootic cycle remains unclear. In this study, we demonstrated that different CspA variants promote spirochete binding to FH to inactivate complement and promote serum resistance in a host-specific manner. Utilizing a tick-to-mouse transmission model, we observed that a cspA-knockout B. burgdorferi is eliminated from nymphal ticks in the first 24 hours of feeding and is unable to be transmitted to naïve mice. Conversely, ectopically producing CspA derived from B. burgdorferi or B. afzelii, but not B. garinii in a cspA-knockout strain restored spirochete survival in fed nymphs and tick-to-mouse transmission. Furthermore, a CspA point mutant, CspA-L246D that was defective in FH-binding, failed to survive in fed nymphs and at the inoculation site or bloodstream in mice. We also allowed those spirochete-infected nymphs to feed on C3-/- mice that lacked functional complement. The cspA-knockout B. burgdorferi or this mutant strain complemented with cspA variants or cspA-L246D was found at similar levels as wild type B. burgdorferi in the fed nymphs and mouse tissues. These novel findings suggest that the FH-binding activity of CspA protects spirochetes from complement-mediated killing in fed nymphal ticks, which ultimately allows Bbsl transmission to mammalian hosts.
Der Basismechanismus immunchemischer Reaktionen ist die Antigen-Antikörper-Reaktion. Die nachzuweisenden Substanzen haben in der Bestimmungsreaktion Antigen-Funktion. Die zur Bestimmung der Substanz
im Test eingesetzten Antikörper können polyklonalen oder monoklonalen Ursprungs sein.
Polyklonal gebildete Antikörper werden technisch gewonnen durch die Reinigung der Immunglobulinfraktion immunisierter Tiere. Trotz mehrerer Reinigungsschritte wird vielfach nicht ein spezifischer Antikörper gewonnen, sondern eine heterogene Antikörperfraktion, die neben dem spezifischen Antikörper weitere Antikörper mit unterschiedlicher Affinität Avidität und Spezifität gegen die immunchemisch zu analysierende Substanz enthält.
Die technische Gewinnung monoklonaler Antikörper verläuft im ersten Schritt ähnlich der auf polyklonalem Wege; ein Tier wird immunisiert mit dem immundiagnostisch zu bestimmenden Antigen. Es wird jedoch nachfolgend nicht Blut des immunisierten Tieres zur Antikörpergewinnung entnommen, sondern die MHz. In vitro werden im nächsten Schritt Antigen-sensibilisierte B-Lymphozyten aus der Milz mit Myelomzellen der Maus unter Bildung von Hybridzellen fusioniert. Die Hybridzellen werden selektioniert und diejenigen cloniert, die den gewünschten spezifischen Antikörper bilden. Auf diesem Wege können Antikörper einheitlicher Affinität, Avidität und Spezifität in größeren Mengen gewonnen werden.
Die Vor- und Nachteile poly- und monoklonal gebildeter Antikörper für immunchemische Tests werden dargestellt.
Biomolecular condensation underlies the biogenesis of an expanding array of membraneless assemblies, including stress granules (SGs) which form under a variety of cellular stresses. Advances have been made in understanding the molecular grammar that dictates the behavior of a few key scaffold proteins that make up these phases but how the partitioning of hundreds of other SG proteins is regulated remains largely unresolved. While investigating the rules that govern the condensation of ataxin-2, a SG protein implicated in neurodegenerative disease, we unexpectedly identified a short 14aa sequence that acts as an ataxin-2 condensation switch and is conserved across the eukaryote lineage. We identify poly(A)-binding proteins as unconventional RNA-dependent chaperones that control this regulatory switch. Our results uncover a hierarchy of cis and trans interactions that fine-tune ataxin-2 condensation and reveal a new molecular function for ancient poly(A)-binding proteins as emulsifiers of biomolecular condensate proteins. These findings may inspire novel approaches to therapeutically target aberrant phases in disease.
We have recently shown that caspase-8 is a new substrate of Polo-like kinase 3 (Plk3) that phosphorylates the protein on residue T273 thereby promoting its pro-apoptotic function. In the present study we aimed to investigate the clinical relevance of Plk3 expression and phosphorylation of caspase-8 at T273 in patients with anal squamous cell carcinoma (SSC) treated with 5-fluorouracil and mitomycin C-based chemoradiotherapy (CRT). Immunohistochemical detection of the markers was performed in pretreatment biopsy specimens of 95 patients and was correlated with clinical/histopathologic characteristics including HPV-16 virus load/p16INK4a expression and cumulative incidence of local and distant failure, cancer specific survival (CSS), and overall survival (OS). We observed significant positive correlations between Plk3 expression, pT273 caspase-8 signal, and levels of HPV-16 virus DNA load/p16INK4a detection. Patients with high scores of Plk3 and pT273 caspase-8 showed increased local control (p = 0.011; p = 0.001), increased CSS (p = 0.011; p = 0.013) and OS (p = 0.024; p = 0.001), while the levels of pT273 caspase-8 were significantly associated (p = 0.033) with distant metastases. In multivariate analyses Plk3 expression remained significant for local failure (p = 0.018), CSS (p = 0.016) and OS (p = 0.023). Moreover, a combined HPV16 DNA load and Plk3 or pT273 caspase-8 variable revealed a significant correlation to decreased local failure (p = 0.001; p = 0.009), increased CSS (p = 0.016; p = 0.023) and OS (p = 0.003; p = 0.003). In conclusion these data indicate that elevated levels of Plk3 and pT273 caspase-8 are correlated with favorable clinical outcome in patients with anal SCC treated with concomitant CRT.
Polo-like kinase 1 regulates the stability of the mitotic centromere-associated kinesin in mitosis

Proper bi-orientation of chromosomes is critical for the accurate segregation of chromosomes in mitosis. A key regulator of this process is MCAK, the mitotic centromere-associated kinesin. During mitosis the activity and localization of MCAK are regulated by mitotic key kinases including Plk1 and Aurora B. We show here that S621 in the MCAK’s C-terminal domain is the major phosphorylation site for Plk1. This phosphorylation regulates MCAK’s stability and facilitates its recognition by the ubiquitin/proteasome dependent APC/CCdc20 pathway leading to its D-box dependent degradation in mitosis. While phosphorylation of S621 does not directly affect its microtubule depolymerising activity, loss of Plk1 phosphorylation on S621 indirectly enhances its depolymerization activity in vivo by stabilizing MCAK, leading to an increased level of protein. Interfering with phosphorylation at S621 causes spindle formation defects and chromosome misalignments. Therefore, this study suggests a new mechanism by which Plk1 regulates MCAK: by regulating its degradation and hence controlling its turnover in mitosis.
Polo-like kinase 1 inhibition sensitizes neuroblastoma cells for vinca alkaloid-induced apoptosis

High polo-like kinase 1 (PLK1) expression has been linked to poor outcome in neuroblastoma (NB), indicating that it represents a relevant therapeutic target in this malignancy. Here, we identify a synergistic induction of apoptosis by the PLK1 inhibitor BI 2536 and vinca alkaloids in NB cells. Synergistic drug interaction of BI 2536 together with vincristine (VCR), vinblastine (VBL) or vinorelbine (VNR) is confirmed by calculation of combination index (CI). Also, BI 2536 and VCR act in concert to reduce long-term clonogenic survival. Importantly, BI 2536 significantly enhances the antitumor activity of VCR in an in vivo model of NB. Mechanistically, BI 2536/VCR co-treatment triggers prolonged mitotic arrest, which is necessary for BI 2536/VCR-mediated apoptosis, since pharmacological inhibition of mitotic arrest by the CDK1 inhibitor RO-3306 significantly reduces cell death. Prolonged mitotic arrest leads to phosphorylation-mediated inactivation of BCL-2 and BCL-XL as well as downregulation of MCL-1, since inhibition of mitotic arrest by RO-3306 also prevents phosphorylation of BCL-2 and BCL-XL and MCL-1 downregulation. This inactivation of antiapoptotic BCL-2 proteins promotes activation of BAX and BAK, cleavage of caspase-9 and -3 and caspase-dependent apoptosis. Engagement of the mitochondrial pathway of apoptosis is critically required for BI 2536/VCR-induced apoptosis, since ectopic expression of a non-degradable MCL-1 phospho-mutant, BCL-2 overexpression or BAK knockdown significantly reduce BI 2536/VCR-mediated apoptosis. Thus, PLK1 inhibitors may open new perspectives for chemosensitization of NB.
Die politische Steuerung des Krankenhaussektors hat sich in den vergangenen anderthalb Jahrzehnten nachhaltig verändert. Das Gesundheitsstrukturgesetz von 1992 markiert einen gesundheitspolitischen Paradigmenwechsel, mit dem verstärkt wettbewerbliche Steuerungsinstrumente in das Gesundheitswesen eingeführt wurden. Auch im stationären Sektor ersetzen bzw. ergänzen wettbewerbliche Instrumente korporatistische Arrangements. Die Gegenüberstellung der politischen Steuerung des Krankenhaussektors vor 1992 mit der Situation nach der Gesundheitsreform 2007 verdeutlicht, dass auf den Feldern Leistungserbringung, Vergütung und Qualitätssicherung sukzessive ein neues Steuerungsmodell entstanden ist. Dieses zeichnet sich durch eine gewachsene Komplexität, eine Zunahme von Steuerungsaktivitäten und eine Neujustierung des Verhältnisses staatlicher bzw. korporatistischer Steuerung einerseits und wettbewerblicher Steuerung andererseits aus. Dort, wo es um allokative Entscheidungen geht, werden korporatistische Elemente durch wettbewerbliche ersetzt. Auf anderen Regulierungsfelder bleibt der Korporatismus dagegen erhalten. Der Staat als „architect of political order“ (Anderson) hat diese Transformation herbeigeführt, sieht sich allerdings zunehmend mit dem Widerspruch zwischen einer bedarfsorientierten Krankenhausplanung und Investitionsfinanzierung auf Landesebene und einer Leistungsverteilung über den Wettbewerb konfrontiert.
The antibody-drug conjugate polatuzumab vedotin (pola) has recently been approved in combination with bendamustine and rituximab (pola-BR) for patients with refractory or relapsed (r/r) large B-cell lymphoma (LBCL). To investigate the efficacy of pola-BR in a real-world setting, we retrospectively analyzed 105 patients with LBCL who were treated in 26 German centers under the national compassionate use program. Fifty-four patients received pola as a salvage treatment and 51 patients were treated with pola with the intention to bridge to chimeric antigen receptor (CAR) T-cell therapy (n = 41) or allogeneic hematopoietic cell transplantation (n = 10). Notably, patients in the salvage and bridging cohort had received a median of 3 prior treatment lines. In the salvage cohort, the best overall response rate was 48.1%. The 6-month progression-free survival and overall survival (OS) was 27.7% and 49.6%, respectively. In the bridging cohort, 51.2% of patients could be successfully bridged with pola to the intended CAR T-cell therapy. The combination of pola bridging and successful CAR T-cell therapy resulted in a 6-month OS of 77.9% calculated from pola initiation. Pola vedotin-rituximab without a chemotherapy backbone demonstrated encouraging overall response rates up to 40%, highlighting both an appropriate alternative for patients unsuitable for chemotherapy and a new treatment option for bridging before leukapheresis in patients intended for CAR T-cell therapy. Furthermore, 7 of 12 patients with previous failure of CAR T-cell therapy responded to a pola-containing regimen. These findings suggest that pola may serve as effective salvage and bridging treatment of r/r LBCL patients.
Macrophages exposed to the Th2 cytokines interleukin (IL) IL-4 and IL-13 exhibit a distinct transcriptional response, commonly referred to as M2 polarization. Recently, IL-4-induced polarization of murine bone marrow-derived macrophages (BMDMs) has been linked to acetyl-CoA levels through the activity of the cytosolic acetyl-CoA-generating enzyme ATP-citrate lyase (ACLY). Here, we studied how ACLY regulated IL-4-stimulated gene expression in human monocyte-derived macrophages (MDMs). Although multiple ACLY inhibitors attenuated IL-4-induced target gene expression, this effect could not be recapitulated by silencing ACLY expression. Furthermore, ACLY inhibition failed to alter cellular acetyl-CoA levels and histone acetylation. We generated ACLY knockout human THP-1 macrophages using CRISPR/Cas9 technology. While these cells exhibited reduced histone acetylation levels, IL-4-induced gene expression remained intact. Strikingly, ACLY inhibitors still suppressed induction of target genes by IL-4 in ACLY knockout cells, suggesting off-target effects of these drugs. Our findings suggest that ACLY may not be the major regulator of nucleocytoplasmic acetyl-CoA and IL-4-induced polarization in human macrophages. Furthermore, caution should be warranted in interpreting the impact of pharmacological inhibition of ACLY on gene expression.
Chronic viral hepatitis is associated with substantial morbidity and mortality worldwide. The aim of our study was to assess the ability of point shear‐wave elastography (pSWE) using acoustic radiation force impulse imaging for the prediction of the following liver‐related events (LREs): new diagnosis of HCC, liver transplantation, or liver‐related death (hepatic decompensation was not included as an LRE). pSWE was performed at study inclusion and compared with liver histology, transient elastography (TE), and serologic biomarkers (aspartate aminotransferase to platelet ratio index, Fibrosis‐4, FibroTest). The performance of pSWE and TE to predict LREs was assessed by calculating the area under the receiver operating characteristic curve and a Cox proportional‐hazards regression model. A total of 254 patients with a median follow‐up of 78 months were included in the study. LRE occurred in 28 patients (11%) during follow‐up. In both patients with hepatitis B virus and hepatitis C virus (HCV), pSWE showed significant correlations with noninvasive tests and TE, and median pSWE and TE values were significantly different between patients with LREs and patients without LREs (both P < 0.0001). In patients with HCV, the area under the receiver operating characteristic curve for pSWE and TE to predict LREs were comparable: 0.859 (95% confidence interval [CI], 0.747‐0.969) and 0.852 (95% CI, 0.737‐0.967) (P = 0.93). In Cox regression analysis, pSWE independently predicted LREs in all patients with HCV (hazard ratio, 17.9; 95% CI, 5.21‐61‐17; P < 0.0001) and those who later received direct‐acting antiviral therapy (hazard ratio, 17.11; 95% CI, 3.88‐75.55; P = 0.0002). Conclusion: Our study shows good comparability between pSWE and TE. pSWE is a promising tool for the prediction of LREs in patients with viral hepatitis, particularly those with chronic HCV. Further studies are needed to confirm our data and assess their prognostic value in other liver diseases.
Insertion of bone substitution materials accelerates healing of osteoporotic fractures. Biodegradable materials are preferred for application in osteoporotic patients to avoid a second surgery for implant replacement. Degraded implant fragments are often absorbed by macrophages that are removed from the fracture side via passage through veins or lymphatic vessels. We investigated if lymphatic vessels occur in osteoporotic bone defects and whether they are regulated by the use of different materials. To address this issue osteoporosis was induced in rats using the classical method of bilateral ovariectomy and additional calcium and vitamin deficient diet. In addition, wedge-shaped defects of 3, 4, or 5 mm were generated in the distal metaphyseal area of femur via osteotomy. The 4 mm defects were subsequently used for implantation studies where bone substitution materials of calcium phosphate cement, composites of collagen and silica, and iron foams with interconnecting pores were inserted. Different materials were partly additionally functionalized by strontium or bisphosphonate whose positive effects in osteoporosis treatment are well known. The lymphatic vessels were identified by immunohistochemistry using an antibody against podoplanin. Podoplanin immunopositive lymphatic vessels were detected in the granulation tissue filling the fracture gap, surrounding the implant and growing into the iron foam through its interconnected pores. Significant more lymphatic capillaries were counted at the implant interface of composite, strontium and bisphosphonate functionalized iron foam. A significant increase was also observed in the number of lymphatics situated in the pores of strontium coated iron foam. In conclusion, our results indicate the occurrence of lymphatic vessels in osteoporotic bone. Our results show that lymphatic vessels are localized at the implant interface and in the fracture gap where they might be involved in the removal of lymphocytes, macrophages, debris and the implants degradation products. Therefore the lymphatic vessels are involved in implant integration and fracture healing.
Background: While the incidence and aspects of pneumonia in ICU patients has been extensively discussed in the literature, studies on the occurrence of pneumonia in severely injured patients are rare. The aim of the present study is to elucidate factors associated with the occurrence of pneumonia in severely injured patients with thoracic trauma.
Setting: Level-I University Trauma Centres associated with the TraumaRegister DGU®.
Methods: A total of 1162 severely injured adult patients with thoracic trauma documented in the TraumaRegister DGU® (TR-DGU) were included in this study. Demographic data, injury severity, duration of mechanical ventilation (MV), duration of ICU stay, occurrence of pneumonia, bronchoalveolar lavage, aspiration, pathogen details, and incidences of mortality were evaluated. Statistical evaluation was performed using SPSS (Version 25.0, SPSS, Inc.) software.
Results: The overall incidence of pneumonia was 27.5%. Compared to patients without pneumonia, patients with pneumonia had sustained more severe injuries (mean ISS: 32.6 vs. 25.4), were older (mean age: 51.3 vs. 47.5) and spent longer periods under MV (mean: 368.9 h vs. 114.9 h). Age, sex (male), aspiration, and duration of MV were all independent predictors for pneumonia occurrence in a multivariate analysis. The cut-off point for duration of MV that best discriminated between patients who would and would not develop pneumonia during their hospital stay was 102 h. The extent of thoracic trauma (AISthorax), ISS, and presence of pulmonary comorbidities did not show significant associations to pneumonia incidence in our multivariate analysis. No significant difference in mortality between patients with and without pneumonia was observed.
Conclusions: Likelihood of pneumonia increases with age, aspiration, and duration of MV. These parameters were not found to be associated with differences in outcomes between patients with and without pneumonia. Future studies should focus on independent parameters to more clearly identify severely injured subgroups with a high risk of developing pneumonia.

Nierentransplantierte Patienten haben im Vergleich zu einer altersgleichen Bevölkerung ein 10-fach erhöhtes Risiko an kardiovaskulären Erkrankungen zu versterben. Graff et al fanden in einer dieser Arbeit vorangegangenen Untersuchung heraus, dass Thrombozyten nierentransplantierter Patienten unter immunsuppressiver Monotherapie voraktiviert sind, verglichen zu einer aus Hypertonikern bestehenden Kontrollgruppe. Des weiteren scheinen „neuere“ Immunsuppressiva wie Tacrolimus (TAC) Thrombozytenaktivierungsmarker weniger stark zu beeinflussen als „ältere“ wie Cyclosporin (CsA). Ziel dieser Arbeit war es den Einfluss einer antithrombozytären Therapie mit Clopidogrel bei nicht antiaggregatorisch vorbehandelten nierentransplantierten Patienten – Gruppe 1 (TAC, n=17) und Gruppe 2 (CsA, n=17) und nierentransplantierten Patienten mit ASS-Dauermedikation – Gruppe 3 (TAC, n=15) und Gruppe 4 (CsA, n=18), welche im Rahmen der Studie auf Clopidogrel umgestellt worden sind, herauszufinden. Alle Patienten wurden vor (V1) und nach einer vierwöchigen Clopidogrel – Therapie (V2) untersucht. Hierzu wurden Parameter, die eine Thrombozytenaktivierung widerspiegeln wie der Degranulationsmarker CD62, der aktivierte GPIIb/IIIa-Rezeptor (PAC1) sowie die Bildung von Plättchen-Leukozyten Aggregaten (CD41) untersucht. Plättchen-Leukozyten Aggregate (PLAs) akkumulieren an Orten mit Gefäßverletzungen und tragen so zu Entzündungsreaktionen und Thrombosen bei. Die Thrombozytenfunktion (CD62, PAC1) und die Formation von PLAs (CD41, CD11b) wurden vor Beginn der Clopidogrel – Therapie (V1, Gruppe 3 und 4 unter ASS – Therapie) sowie nach vierwöchiger Clopidogrel – Behandlung (V2) durchflusszytometrisch und aggregometrisch untersucht. Bei V1 wiesen CsA behandelte Patienten (Gruppe 2) signifikant (p<0,05) höhere Thrombozytenaktivierungsmarker auf als mit TAC behandelte Patienten: CD62 (185 [112-223] vs. 110 [84-173], median [1Q-3Q]), PAC1 (27 [26-38] vs. 15 [12-35]) und PLA (430 [342-769] vs. 237 [177-510]). Nach vierwöchiger Clopidogrel – Therapie (V2) kam es zu einer signifikanten Reduktion der untersuchten Parametern: CD62 (95 [82-123] Gruppe 2, 71 [56-94] Gruppe 1), PAC1 (15 [8-22] Gruppe 2, 12 [3-15] group 1), PLA (269 [225-472] Gruppe 2, 194 [159-275] Gruppe 1). Bei nierentransplantierten Patienten mit vorheriger ASS – Comedikation (Gruppe 3 und 4) konnte hinsichtlich der Thrombozytenaktivierung zwischen CsA und TAC bei V1 kein signifikanter Unterschied festgestellt werden. Es ließ sich allerdings ein Trend zugunsten TAC ablesen: CD62 (165 [96-289] vs. 111 [73-156]), PAC1 (19 [12-39] vs. 14 [8-32]), und PLA (437 [232-852] vs. 282 [164-471]). Die Umstellung von ASS auf Clopidogrel führte bei CsA behandelten Patienten (Gruppe 4) zu einer signifikanten Reduktion von CD62, PAC1 und der Formation von PLAs bei V2. TAC behandelten Patienten zeigten dagegen nur eine signifikante Abnahme des PAC1-Markers; für CD62 und PLAs wurden zwar bei V2 niedrigere Werte als bei V1 gemessen, allerdings erreichten die Unterschiede keine statistische Signifikanz: CD62 (90 [45-139] Gruppe 4, 67 [51-116] Gruppe 3), PAC1 (11 [7-19] Gruppe 4, 10 [5-20] Gruppe 3), PLA (285 [186-470] Gruppe 4, 250 [143-298] Gruppe 3). Zusammenfassend lässt sich ableiten, dass CsA die Thrombozytenaktivierung sowie die Formation von PLAs stärker negativ beeinflusst als TAC. Clopidogrel scheint diesen Vorgang effektiver als Aspirin zu hemmen.
Immunsuppressiva sind essentiell für die allogene Organtransplantation. Graff et al konnten in drei vorangegangenen Studien zeigen, dass Immunsuppressiva zu einer verstärkten Thrombozytenaktivierung führen. Diese ist für die verschiedenen Substanzen unterschiedlich stark ausgeprägt. Eine verstärkte Thrombozytenaktivierung könnte zu vermehrten kardiovaskulären Ereignissen führen. Insbesondere bei nierentransplantierten Patienten ist das kardiovaskuläre Risiko ohnehin deutlich erhöht. Es stellt sich die Frage ob diese medikamentös induzierte Plättchenaktivierung als Ko-Faktor kardiovaskulärer Ereignisse bei nierentransplantierten anzusehen ist. Im Vorfeld dieser Studie wurden in den Studien NTX, RAPA und PLANT bei insgesamt 176 nierentransplantierten Patienten mit einer Mono-Immunsuppression flowzytometrische Plättchenfunktionsmessungen durchgeführt, dabei wurden insbesondere der Degranulationsmarker CD62 und der aktivierte GP IIb/IIIa-Rezeptor PAC-1 gemessen. Diese Ergebnisse wurden in Relation zum Mittelwert der jeweiligen Kontrollgruppen gesetzt: Messwert(Patient) / Mittelwert(Gesunde_Kontrolle) Diese relativen Plättchenfunktionswerte waren zwischen den Studien vergleichbar. Des Weiteren wurden epidemiologische Eckdaten und klinische Endpunkte bis zu 5 Jahre nach der Thrombozytenmessung erfasst, im Mittel 3,7 Jahre. Aufgrund des Studiendesigns der vorangegangenen Studien wurde hier ein Kollektiv mit relativ niedrigem kardiovaskulärem Risiko untersucht, jedoch sollte durch diese Reduktion klassischer Risikofaktoren der Einfluss der Plättchenaktivität besser sichtbar sein. Die retrospektive Analyse untersuchte ob die Plättchenfunktionsparameter CD62 und PAC-1 als prädiktiver Marker für kardiovaskuläre Ereignisse geeignet sind. Insgesamt traten seit den Messungen 21 kardiovaskuläre Ereignisse auf, davon 9 akute. Es gab keine signifikanten Unterschiede zwischen den Werten bei Patienten mit Ereignissen und denen ohne Ereignis: Bei CD62 (baseline) lag der Mittelwert bei 2,82±1,9 mit Ereignis vs. 2,73±1,47 ohne, für PAC-1 bei 1,76 ±1,97 vs. 1,93±1,53. Nach Aktivierung mit TRAP lag der Mittelwert für CD62 bei Ereignispatienten bei 1,57±0,88 vs. 1,76±0,83, für PAC-1 bei 1,56±1,65 vs. 1,44±1,04. In einem weiteren Schritt wurden die Patienten nach dem Ausmaß der Plättchenaktivierung klassifiziert, als Cut-Off-Werte wurden die Mittelwerte und Mediane der jeweiligen Plättchenfunktion aller Patienten gewählt, ferner noch willkürlich eine 3- bzw 5-fache Aktivierung gegenüber den gesunden Kontrollen. Hierbei wurden in den Gruppen mit hohen Funktionswerten nicht signifikant mehr Ereignisse beobachtet als in den Gruppen mit niedrigen Werten. In einer daraufhin durchgeführten Time-to-event-Analyse nach Kaplan-Meyer zeigte sich allerdings für CD62 (baseline) > 5 ein früheres Eintreten kardiovaskulärer Ereignisse, im Mittel 47,6 vs. 69,9 Monate, p = 0,05. Als Risikofaktoren in dieser Studie erwiesen sich insbesondere das Alter und das Alter bei Transplantation, letzteres erwies sich in einer logistischen Regressionsanalyse als der wichtigster Faktor. Dies ist nach unserem Wissen die erste Untersuchung, die den prädiktiven Wert von Thrombozyten-gebundenem CD62 sowie von PAC-1 für kardiovaskuläre Ereignisse evaluiert. Es ergibt sich kein Hinweis, dass diese Marker einen prädiktiven Wert haben. Somit erscheint auch eine direkte pharmakologische Beeinflussung der Thrombozytenfunktion oder eine Veränderung der Immunsuppression nicht wegweisend, um die Inzidenz kardiovaskulärer Ereignisse bei nierentransplantierten Patienten zu senken.

The spindle assembly checkpoint (SAC) acts as a molecular safeguard in ensuring faithful chromosome transmission during mitosis, which is regulated by a complex interplay between phosphatases and kinases including PLK1. Adenomatous polyposis coli (APC) germline mutations cause aneuploidy and are responsible for familial adenomatous polyposis (FAP). Here we study the role of PLK1 in colon cancer cells with chromosomal instability promoted by APC truncation (APC-ΔC). The expression of APC-ΔC in colon cells reduces the accumulation of mitotic cells upon PLK1 inhibition, accelerates mitotic exit and increases the survival of cells with enhanced chromosomal abnormalities. The inhibition of PLK1 in mitotic, APC-∆C-expressing cells reduces the kinetochore levels of Aurora B and hampers the recruitment of SAC component suggesting a compromised mitotic checkpoint. Furthermore, Plk1 inhibition (RNAi, pharmacological compounds) promotes the development of adenomatous polyps in two independent ApcMin/+ mouse models. High PLK1 expression increases the survival of colon cancer patients expressing a truncated APC significantly.
RBFOX1 is a highly pleiotropic gene that contributes to several psychiatric and neurodevelopmental disorders. Both rare and common variants in RBFOX1 have been associated with several psychiatric conditions, but the mechanisms underlying the pleiotropic effects of RBFOX1 are not yet understood. Here we found that, in zebrafish, rbfox1 is expressed in spinal cord, mid- and hindbrain during developmental stages. In adults, expression is restricted to specific areas of the brain, including telencephalic and diencephalic regions with an important role in receiving and processing sensory information and in directing behaviour. To investigate the effect of rbfox1 deficiency on behaviour, we used rbfox1sa15940, a rbfox1 loss-of-function line. We found that rbfox1sa15940 mutants present hyperactivity, thigmotaxis, decreased freezing behaviour and altered social behaviour. We repeated these behavioural tests in a second rbfox1 loss-of-function line with a different genetic background, rbfox1del19, and found that rbfox1 deficiency affects behaviour similarly in this line, although there were some differences. rbfox1del19 mutants present similar thigmotaxis, but stronger alterations in social behaviour and lower levels of hyperactivity than rbfox1sa15940 fish. Taken together, these results suggest that rbfox1 deficiency leads to multiple behavioural changes in zebrafish that might be modulated by environmental, epigenetic and genetic background effects, and that resemble phenotypic alterations present in Rbfox1-deficient mice and in patients with different psychiatric conditions. Our study thus highlights the evolutionary conservation of rbfox1 function in behaviour and paves the way to further investigate the mechanisms underlying rbfox1 pleiotropy on the onset of neurodevelopmental and psychiatric disorders.

FTY720, ein synthetisches Sphingosin Analogon von Myriocin, ist ein neuartigerImmunomodulator, der in vivo Homing und Migration von Lymphozyten moduliertund hierbei die Immunantwort in verschiedenen Transplantations- undAutoimmunmodellen hemmt. Zudem konnte in aktuellen Untersuchungen einausgeprägter Anti-Tumor-Effekt von FTY720 in mehreren Krebszelllinien,einschließlich Leukämie-, Lymphoma- und Glioma-Zellen dargelegt werden. In vitroVersuche zeigten, dass FTY720 die Proliferation von malignen Zellen hemmtund/oder Apoptose in Krebszellen induziert. Im Rahmen dieser Dissertationuntersuchten wir zum ersten Mal die biologischen Wirkungen von FTY720 inhumanen Kolonkarzinomzellen.
Zunächst wurde die Auswirkung von FTY720 auf das Wachstumsverhalten von dreiverschiedenen Kolonkarzinomzelllinien (Caco-2, HT-29 und HCT-116) durch dieMessung der Zell-Proliferation mittels BrdU-Einbau und Kristallviolett-Assay sowiederStoffwechselaktivität im MTT-Test ermittelt. Die Behandlung dieser Zelllinienmit steigenden Konzentrationen von FTY720 (6-15 μM) führte zu einer dosis- undzeitabhängigen Verminderung der Zell-Überlebensrate. Zur Charakterisierung dieserHemmung der Proliferation untersuchten wir mit Hilfe von Western blot Analysen dieExpression wichtiger Faktoren der Zellzykluskontrolle in Caco-2-Zellen unterWirkung von FTY720. Dabei konnten wir zeigen, dass die Expression des Cdk-Inhibitor p27Kip1 hochreguliert wird, während die von Cyclin D1 und Cdk4 starkreduziert werden, was auf einen Zellzyklusarrest in der G1-Phase hindeutet. Dienachfolgende Untersuchung hinsichtlich einer Induktion von Apoptose zeigte eineSteigerung der DNA-Fragmentierung nach Behandlung mit FTY720, begleitet voneiner Zunahme der Bax/Bcl-2-Ratio als Zeichen einer Aktivierung desmitochondrialen (intrinsischen) Weges, und letztendlich einer Aktivierung vonCaspase-3. Ferner stellten wir fest, dass FTY720 zu einer signifikanten Steigerung derTRAIL-Protein-Expression führt und so möglicherweise Apoptose ebenfalls über denextrinsischen Signalweg vermittelt. Schließlich konnte gezeigt werden, dass der Anti-Tumor-Effekt von FTY720 mit einer Hemmung der Phosphorylierung vonp44/42MAPK, terminalem Stellglied des Ras/MAPK-Signalweges, und einer Induktion der Expression von PTEN, einem negativem Regulator des PI3K/Akt-Signalweges,einhergeht.
Zusammenfassend ist festzustellen, dass FTY720 in vitro im kolorektalen Karzinomeinen ausgeprägten Anti-Tumor-Effekt besitzt, welcher durch eine Inhibition desZellwachstums über einen G1-Zellzyklusarrest und eine Induktion der Apoptose überden extrinsischen und den intrinsischen Weg vermittelt wird.
Platypnea orthodeoxia is a rare disorder characterized by dyspnea and arterial desaturation, exacerbated by the upright position and relieved when the subject is recumbent. We report the case of a 79-year old woman admitted to hospital with dyspnea who was thought to have restrictive ventilatory impairment due to osteoporosis and severe kyphosis. Interestingly, the dyspnea was aggravated in the upright position, whereas the symptoms improved in the supine position. Arterial blood gas analysis confirmed orthodeoxia. The lung function test showed only a mild obstructive and restrictive ventilation disorder. Echocardiography revealed a patent foramen ovale and an aneurysm of the atrial septum protruding into the left atrium, despite normal right atrial pressure. Transesophageal echocardiography showed a prominent Eustachian valve guiding a blood flow from the inferior vena cava directly onto the atrial septum, thereby pushing open the patent foramen ovale. Contrast-enhanced echocardiography confirmed a spontaneous right-to-left shunt through the patent foramen ovale. It was assumed that the platypnea-orthodeoxia was caused by a prominent Eustachian valve redirected to the patent foramen ovale as a result of severe osteoporosis with subsequent thoracic kyphosis and a change in the position of the entire heart. The patient underwent permanent transcatheter closure of the patent foramen ovale after hemodynamic assessment had confirmed a significant right-to-left shunt through it. After the procedure the arterial oxygen pressure increased significantly in the upright position and dyspnea improved.
BACKGROUND: Platinum-based drugs are used as cancer chemotherapeutics for the last 40 years. However, drug resistance and nephrotoxicity are the major limitations of the use of platinum-based compounds in cancer therapy. Platinum (IV) complexes are believed to act as platinum prodrugs and are able to overcome some of platinum (II) limitations.
METHODS: A number of previously sensitized platinum (IV) complexes were evaluated for their anti-cancer activity by monitoring ability to affect proliferation, clonigenicity and apoptosis induction of Cisplatin sensitive and resistant cancer cells. In addition, the uptake of Cisplatin and the platinum (IV) derivatives to Cisplatin sensitive and resistant cancer cells was monitored.
RESULTS: The bis-octanoatoplatinum (IV) complex (RJY13), a Cisplatin derivative with octanoate as axial ligand, exhibited strong anti-proliferative effect on the Cisplatin resistant and sensitive ovarian cells, A2780cisR and A2780, respectively. Moreover, RJY13 exhibited good activity in inhibiting clonigenicity of both cells. Anti-proliferative activity of RJY13 was mediated by induction of apoptosis. Interestingly, a bis-lauratopaltinum (IV) complex (RJY6) was highly potent in inhibiting clonigenicity of both Cisplatin sensitive and Cisplatin resistant cells, however, exhibited reduced activity in assays that utilize cells growing in two dimensional (2D) conditions. The uptake of Cisplatin was reduced by 30% in A2780 in which the copper transporter-1 (Ctr1) was silenced. Moreover, uptake of RJY6 was marginally dependent on Ctr1, while uptake of RJY13 was Ctr1-independent.
CONCLUSIONS: Our data demonstrated the potential of platinum (IV) prodrugs in overcoming acquired and inherited drug resistance in cancer cell lines. Moreover, our data demonstrated that the uptake of Cisplatin is partially dependent on Ctr1 transporter, while uptake of RJY6 is marginally dependent on Ctr1 and RJY13 is Ctr1-independent. In addition, our data illustrated the therapeutic potential of platinum (IV) prodrugs in cancer therapy.
Ziel. Lokoregionäre Rezidive sind der Hauptgrund für ein Therapieversagen nach primärer multimodaler Behandlung von Plattenepithelkarzinomen der Kopf-Hals-Region (SCCHN). Wir verglichen die Effektivität und Toxizität von Cisplatin oder Cetuximab simultan zur Re-Bestrahlung (ReRT) bei inoperablen SCCHN-Rezidiven. Ein prognostischer Score sollte auf Grundlage verschiedener klinischer und pathologischer Faktoren etabliert werden.
Patienten und Methoden. 66 Patienten mit in vorbestrahlten Regionen rezidivierten SCCHN wurden von 2007 bis 2014 simultan mit Cetuximab (n=33) oder cisplatin-basierter Chemotherapie (n=33) re-bestrahlt. Die Toxizität wurde wöchentlich sowie bei jedem Nachsorgetermin erfasst. Klinische Untersuchung, Endoskopie, CT- oder MRT-Untersuchungen wurden zur Beurteilung des Therapieansprechens und der Krankheitskontrolle eingesetzt.
Ergebnisse. Nach einer mittleren Nachbeobachtungszeit von 18,3 Monaten betrug das 1-Jahres-Überleben (OS) für ReRT mit Cetuximab 44,4% und mit cisplatin-basierter Chemotherapie 45,5% (p=0.352). Die lokalen Kontollraten nach einem Jahr waren jeweils 46,4% und 54,2% (p=0.625); die Raten an Metastasenfreiheit 73,6% und 81% (p=0.842). Hämatologische Toxizität ≥ Grad 3 kam in der Cisplatin-Gruppe häufiger vor (p<0.001), dagegen trat Schmerz ≥ Grad 3 in der Cetuximab-Gruppe häufiger auf (p=0.034). Ein physiologischer Hb-Wert und ein längeres Intervall zwischen primärer RT und ReRT erwiesen sich als signifikante prognostische Faktoren für das OS (multivariat: p=0.003 und p=0.002). Die Rezidivlokalisation sowie das GTV zeigten keinen signifikanten Einfluss auf das OS in der multivariaten Analyse (p=0.160 und p=0.167). Ein auf Grundlage dieser Variablen konstruierter Prognose-Score (0 bis 4 Punkte) zeigte signifikante Überlebensunterschiede: 1-Jahres-OS für 0/1/2/3/4 Prognosepunkte: 10%, 38%, 76%, 80% und 100% (p<0.001).
Schlussfolgerung. Sowohl Cetuximab- als auch Cisplatin-basierte ReRT für SCCHN-Rezidive sind gut durchführbare und effektive Behandlungsoptionen mit vergleichbaren Ergebnissen bezüglich Tumorkontrolle und Überleben. Die akuten Nebenwirkungen könnten gering variieren. Unser Prognose-Score könnte zur Identifizierung der für ReRT geeigneten Patienten sowie zur Stratifizierung in künftigen klinischen Studien dienen.
