Development of treatment strategies of chronic inflammatory disorders relies on on-going progress in drug discovery approaches and related molecular biologics. This study presents a gene reporter-based approach of phenotypic screening for anti-inflammatory compounds in the context of rheumatoid arthritis (RA).
CEBPD gene, used as the target gene for the screening readout, encodes CCAAT/enhancer binding protein delta (C/EBPδ) transcription factor (TF). Structural and regulatory characteristics of CEBPD gene as well as function of C/EBPδ TF in the context of inflammation satisfied assay requirements. C/EBPδ TF acts as a key regula-tor of inflammatory gene transcription in macrophages (Mϕ) and is observed to con-tribute to disease development in both a rodent model of RA and RA patient biopsies.
Despite well-described pro-inflammatory effects of C/EBPδ TF, it functions as a cell context-specific signal integrator showing also an anti-inflammatory activity. Conse-quently, both activation and inhibition of CEBPD alike may display a desired anti-inflammatory effect. The aim of this study was to develop a high-throughput screening assay for
CEBPD-modulating compounds and confirm hit compounds’ anti-inflammatory effects via gene expression analysis.
Generation and characterization of a multi-gene-reporter cassette 1.0 encoding enzy-matic secreted alkaline phosphatase (SEAP) gene reporter was a priority during the assay development. Chemiluminescent SEAP assay demonstrating high assay sensitivi-ty, broad linear range, high reproducibility and repeatability was chosen to monitor activity of the defined CEBPD promoter (CEBPD::SEAP). PMA-differentiated and M1-polarized THP-1-derived Mϕ stably expressing multi-gene-reporter cassette 1.0 were used as the assay’s cellular system. mRNA expression of both reporter CEBPD::SEAP and endogenous CEBPD mirrored each other in response to a LPS and IFN-g-triggered inflammatory stimulus (M1 treatment), even though the defined CEBPD promoter re-gion, utilized in the assay, contained only the most proximal and known regulatory se-quences. SEAP chemiluminescence in the reporter cells´ supernatant reliably correlat-ed with the M1 treatment-induced CEBPD::SEAP gene expression. The final screening protocol was developed for semi-automatic screening in the 384-well format.
In total, 2054 compounds from LOPAC®1280 and ENZO®774 libraries were screened twice
using the enzymatic SEAP readout with subsequent analysis of 18 selected compounds: nine with the highest and nine with the lowest signals, further characterized by qPCR. Gene expression levels of endogenous CEBPD, CEBPD::SEAP reporter as well as, IL-6,
IL-1β, and CCL2 as inflammatory markers were quantified. qPCR assays failed to corre-late to SEAP readout in 15 compounds within three standard deviations (SDs) from sol-vent control: nine low signal and six high signal compounds. Demonstrating both assay sensitivity and specificity, a correlation between qPCR gene expression and SEAP readout was observed for three hit compounds with signals above three SDs: BET inhib-itors (BETi) GSK 1210151A and Ro 11-1464 as well as an HDAC inhibitor (HDACi) vori-nostat. The control compound trichostatin A (TSA) that reproducibly upregulated SEAP readout is also an HDAC inhibitor with a similar structure to vorinostat and was there-fore included in the anti-inflammatory phenotype analysis.
The observed suppression of IL-6, IL-1ß, and CCL2 gene expression by hit compounds suggested their anti-inflammatory effect in THP-1 reporter Mϕ. mRNA expression of
IL-6 and CCL2 was suppressed by HDACi and BETi at both 4 and 24 hours, while BETi reduced IL-1β mRNA expression 24 hour time point. BETi significantly upregulated gene expression of both reporter CEBPD::SEAP and endogenous CEBPD, 4 hours after M1 treatment. At the same time point, HDACi completely abolished the mRNA expres-sion of the endogenous CEBPD, while simultaneously upregulating mRNA expression of the reporter CEBPD::SEAP. The use of the most proximal 300 base pairs region of en-dogenous CEBPD promoter, making the upstream regulatory elements unavailable in the assay, may account for differential expression levels of SEAP and C/EBPδ TF. This observation corroborated the need to include a longer and more extensive CEBPD´s gene regulatory area. Thus, an improved multi-gene-reporter cassette 2.0 was gener-ated to be used on the basis of a bacterial artificial chromosome (BAC) covering CE-BPD´s genomic area of about 200,000 base pairs.
The generated screening assay is flexible, reliable, and sensitive displaying potential for drug discovery and drug repurposing. The pharmacological modulation of CEBPD gene expression, first reported for GSK 1210151A, Ro 11-1464, and vorinostat, contrib-utes to the understanding of inflammatory responses in Mϕ and may have RA thera-peutic applications.
Hypertonie stellt in der westlichen Welt die Haupttodesursache dar, obwohl sie im Hinblick auf die pharmakologischen Therapieoptionen gut behandelbar ist. Hauptursächlich ist hierfür eine, vor allem durch eine Non-Adhärenz (u.a. bedingt durch den asymptomatischen Charakter) und in selteneren Fällen eine aufgrund einer therapieresistente Hypertonie (TRH) verursachte, unzureichende Blutdruckkontrolle. Eine Möglichkeit zur Überprüfung der Therapietreue in der antihypertensiven Therapie ist der qualitative Nachweis der Arzneistoffe im Blut oder Urin. Unklar ist, inwiefern die Substanzen in einer biologischen Probe innerhalb des Dosierungsintervalls oder darüber hinaus nachweisbar sind und es zu einer Falschbeurteilung kommen kann.
Daher wurde eine quantitative chromatographisch-tandem-massenspektrometrische Methode für das Therapeutische Drug Monitoring von blutdrucksenkenden Arzneistoffen entwickelt und analytisch vollständig validiert. Bei 38 Patienten mit überwachter Medikamenteneinnahme wurde die Aussagekraft der Methode hinsichtlich der Bestätigung einer Adhärenz zunächst mittels zweier Wirkstoffkonzentrationen im Serum (Tal- und Spitzenspiegel) überprüft. Zur Bewertung der Konzentrationen wurden zwei Konzepte evaluiert. Einerseits wurde die untere Grenze des therapeutischen Referenzbereiches (TRR, Literaturdaten) und andererseits die mittels Rechenmodell (Daten pharmakokinetischer Studien) individuell ermittelte, dosisbezogene Arzneimittelkonzentration (DRC) evaluiert. In einem zweiten Studienansatz wurde diese neue quantitative Methode an einem Kollektiv von 36 ambulanten Patienten (ohne überwachte Medikamenteneinnahme) angewendet und zur Überprüfung der Aussagekraft mit Ergebnissen des etablierten Urinscreenings verglichen.
Die gemessenen Wirkstoffkonzentrationen von Atenolol (64 bis 564 ng/ml), Bisoprolol (2,5 bis 53 ng/ml), Metoprolol (5,8 bis 110 ng/ml), Nebivolol (0,32 bis 3,4 ng/ml), Hydrochlorothiazid (15 bis 606 ng/ml), Furosemid (22 ng/ml), Torasemid (17 bis 1829 ng/ml), Canrenon (25 bis 221 ng/ml), Amlodipin (2,4 bis 35 ng/ml), Lercanidipin (0,24 bis 21 ng/ml), Candesartan (6,0 bis 268 ng/ml), Telmisartan (22 bis 375 ng/ml) und Valsartan (115 bis 7962 ng/ml) haben gezeigt, dass die quantitative Analyse von Antihypertensiva in Serumproben und deren Auswertung auf Basis der individuell berechneten unteren DRC in der Beurteilung einer Adhärenz vielversprechend ist.
Die Auswertung auf Basis der unteren Grenze des TRR signalisierte bei den stationären Patienten (überwachte Einnahme) innerhalb der Substanzklasse der Diuretika (ohne Torasemid) bei 16,7 %, der β-Blocker bei 29,4 %, der Calciumkanal-Blocker bei 14,8 % und der AT1-Antagonisten bei 25 % fälschlicherweise eine Non-Adhärenz.
Die rein qualitative Urinanalyse zeigte im Falle der β-Blocker Atenolol, Bisoprolol und des Diuretikums HCT aufgrund einer hohen Bioverfügbarkeit, einer langen Halbwertszeit oder einer überwiegend renalen Ausscheidung der Muttersubstanz ein Nachweisfenster über das Dosierungsintervall hinaus, was bedingt, dass einige Patienten fälschlicherweise als adhärent gewertet wurden. Ein anderes Problem zeigte sich bei einigen Patienten, die mit dem AT1-Antagonist Candesartan oder dem Calciumkanal-Blocker Lercanidipin behandelt wurden und die als non-adhärent eingestuft wurden. Eine geringe Bioverfügbarkeit, eine hohe Metabolisierungsrate oder geringe renale Ausscheidung der unveränderten Arzneistoffe lies auf eine mangelhafte Nachweisbarkeit innerhalb des Dosierungsintervalls und damit auf eine eingeschränkte Beurteilbarkeit schließen.
Aus den Untersuchungen ergibt sich, dass es bei Anwendung qualitativer Nachweismethoden aufgrund besonderer Pharmakokinetiken einzelner antihypertensiver Wirkstoffe zur Fehleinschätzung der Adhärenz kommen kann. Die neu entwickelte Methodik in Form einer quantitativen Serumanalyse ist unter Verwendung patientenindividueller Bewertungskriterien bei dieser Fragestellung überlegen.
The N-terminus of the hepatitis B virus (HBV) large surface protein (LHB) differs with respect to genotypes. Compared to the amino terminus of genotype (Gt)D, in GtA, GtB and GtC, an additional identical 11 amino acids (aa) are found, while GtE and GtG share another similar 10 aa. Variants of GtB and GtC affecting this N-terminal part are associated with hepatoma formation. Deletion of these amino-terminal 11 aa in GtA reduces the amount of LHBs and changes subcellular accumulation (GtA-like pattern) to a dispersed distribution (GtD-like pattern). Vice versa, the fusion of the GtA-derived N-terminal 11 aa to GtD causes a GtA-like phenotype. However, insertion of the corresponding GtE-derived 10 aa to GtD has no effect. Deletion of these 11aa decreases filament size while neither the number of released viral genomes nor virion size and infectivity are affected. A negative regulatory element (aa 2–8) and a dominant positive regulatory element (aa 9–11) affecting the amount of LHBs were identified. The fusion of this motif to eGFP revealed that the effect on protein amount and subcellular distribution is not restricted to LHBs. These data identify a novel region in the N-terminus of LHBs affecting the amount and subcellular distribution of LHBs and identify release-promoting and -inhibiting aa residues within this motive.
Purpose: We evaluated efficacy and safety profile of patients with anticoagulation therapy (AT) undergoing holmium laser enucleation of the prostate (HoLEP).
Methods: Within our prospective institutional database (11/2017 to 11/2019), we analyzed functional outcomes and 30-day complication rates of HoLEP patients according to Clavien–Dindo classification (CLD), stratified according to specific AT vs. no AT. Further analyses consisted of uni- and multivariate logistic regression models (LRM) predicting complications.
Results: Of 268 patients undergoing HoLEP, 104 (38.8%) received AT: 25.7% were treated with platelet aggregation inhibitors (PAI), 8.2% with new oral anticoagulants (NOAC) and 4.9% with AT-combinations or coumarins bridged with low molecular weight heparins (LMWH/combination). Patients receiving AT were significantly more comorbid (p < 0.01). Pre- and postoperative maximal flow rates, residual void urine and IPSS at 3 months after surgery were invariably improved after HoLEP for patients with/ without AT. Overall complication rate was 19.5% in patients with no AT vs. 26.1% vs. 27.3 vs. 46.2%, respectively, in patients with PAI, NOAC and LMWH/combination (p < 0.01). Major complications (CLD ≥ 3b) occurred in 6.1% of no AT patients vs. 4.3% vs. 4.5 vs. 0% in patients with PAI, NOAC and LMWH/combination, respectively (p < 0.01). In multivariate LRM, AT was not significantly associated with higher complication rates, whereas high ASA status (OR 2.2, p = 0.04), age (OR 1.04, p = 0.02) and bioptical or incidental prostate cancer (OR 2.5, p = 0.01) represented independent risk factors.
Conclusion: Despite higher overall complication rates in AT patients, major complications were not more frequent in AT patients. HoLEP is safe and effective in anticoagulated patients.
Das PCOS ist die häufigste Endokrinopathie fertiler Frauen, die mit Infertilität und Metabolischem Syndrom ernst zu nehmende und belastende Konsequenzen haben kann. Vor dem Hintergrund einer bis heute teilweise unklaren Ätiopathogenese hat das PCOS umfassende Auswirkungen auf den Metabolismus und die Reproduktion. Aufgrund der phänotypischen Variabilität ist die Festlegung einheitlicher diagnostischer Kriterien ständiges Thema in der PCOS-Forschung. Die Rotterdam-Kriterien gelten aktuell als angemessenste Definition. Die Einführung einer aktuali- sierten Nomenklatur zur Optimierung der Diagnosefindung wird diskutiert. Die Therapie beläuft sich bisher auf symptomatische Ansätze.
Ziel dieser Arbeit war es, erstmals eine Analyse der weltweiten Forschung zum PCOS mittels szientometrischer Parameter durchzuführen. Die Erfassung der Daten aus dem WoS belief sich auf die Jahre 1900 – 2015 und ergab 6.262 Artikel zum Thema PCOS.
Die meisten Publikationen kamen aus den USA an erster, Großbritannien an zweiter und Italien an dritter Stelle. Diese drei Länder erhielten in gleicher Platzierung die meisten Zitierungen und erreichten die höchsten modifizierten h-Indizes. Bei Betrachtung der Zr standen jedoch die Niederlande aufgrund der dort veröffentlichten Rotterdam-Kriterien an erster Stelle. Im Verhältnis zur Einwohnerzahl publizierte Griechenland die meisten Artikel, gefolgt von Finnland und Slowenien. Bei Erstellung eines Quotienten aus Artikelanzahl und BIP lag Serbien vor Griechenland und Slowenien. Bezüglich der Artikel pro BIP p.c. fiel China auf den ersten Rang, danach die Türkei und die USA.
Bei den Fachzeitschriften rangierte das Journal of Clinical Endocrinology & Metabolism bezüglich des IF, der Zr und der Anzahl der Zitierungen an erster Stelle. Fertility & Sterility publizierte über die Zeit die meisten Artikel zum Thema PCOS und erlangte die zweithöchste Zahl an Zitierungen.
Die chronologische Analyse zeigt eine Abnahme der Zuteilung von Artikeln zum Fachgebiet Obstetrics & Gynecology und eine Zunahme von Reproductive Biology, während Endocrinology & Metabolism einen gleichbleibenden Anteil hat. Der Anteil kleinerer Fächer wie Research & Experimental, Nutrition & Dietetics und Pharmacy nimmt über die letzten zehn Jahre zu.
Bezüglich der Anzahl an Institutionen liegen die Türkei und Großbritannien auf Platz zwei und drei hinter den USA. Die produktivsten Forschungseinrichtungen sind die University of London, die Pennsylvania State University und die University of Athens. In der italienischen Forschungsgemeinde finden sich die meisten interinstitutionellen Kooperationen.
Die meisten Artikel, auch in Erst- und Koautorenschaft, hat R.S.Legro publiziert. S. Franks führt die Liste der Seniorautoren an. A. Dunaif erreicht bei Zitierungen, Zitationsrate und modifiziertem h-Index herausragende Werte. Wie schon bei den Institutionen liegen italienische Autoren bei der Kooperationsanalyse vorne. Unter den zehn produktionsstärksten Institutions- kooperationen finden sich mit einer griechischen und einer US-amerikanischen Liaison nur zwei nicht italienischen Ursprungs. Zum Zitierverhalten ist zu sagen, dass fast 50 % der Zitierungen Selbstzitierungen sind.
Generell dominieren Männer noch die Welt der Wissenschaft. Dies ist, in Bezug auf die PCOS-Forschung, zu jedem Zeitpunkt so gewesen und spiegelt sich bis heute in den Analysen wider. Seit 2008 ist allerdings eine Annäherung der Werte zu beobachten.
Es kristallisiert sich die Notwendigkeit einer klaren Definition und aktualisierter Nomenklatur des PCOS heraus, um die leicht stagnierende Forschung des komplexen Krankheitsbildes, dessen Ätiopathogenese nicht zur Gänze erklärt ist, anzustoßen. Überraschenderweise treten Nationen wie die Türkei, Griechenland und der Iran bei den Analysen hervor und reichen mit Produktivität und Resonanz an Staaten wie die USA und Großbritannien heran, übertreffen sie mitunter. Dies ist möglicherweise nationalen Besonderheiten, wie erhöhter Prävalenz des Syndroms bzw. erhöhtem sozialen Druck durch das Kardinalsymptom der Infertilität in diesen Ländern, geschuldet. Eine Dominanz an weiblichen Forscherinnen zeigt sich überdies im Iran und ist sonst in nur wenigen Ländern, darunter die USA, zu finden. Männliche Vorherrschaft ist also weniger an bestimmte Regionen, als vielmehr an nationale Strukturen gebunden.
Duale Thrombozytenaggregationshemmung (‚dual antiplatelet therapy‘: DAPT) erhöht das Risiko für eine hämorrhagische Transformation (HT) von ischämischen Schlaganfällen nach Thrombolyse mit gewebespezifischem Plasminogenaktivator (‚tissue plasminogen activator‘: tPA). Bisherige klinische Studien waren jedoch nicht vollends eindeutig, ob diese erhöhte Blutungswahrscheinlichkeit tatsächlich zu einer schlechteren Ausgangssituation für Patienten führt. Viele sehen die initiale klinische Verschlechterung im Rahmen einer potenziellen HT durch den Nutzen der wiederhergestellten Rekanalisation verschlossener Gefäße aufgewogen. Aus diesem Grunde sollte tPA auch in Patienten angewendet werden, die einen Schlaganfall unter DAPT erleiden. Bisher sind der Pathomechanismus und die beteiligten Mediatoren der HT unverstanden. Allerdings könnte die Reduktion der tPA-assoziierten HT zu einer sichereren Anwendung der Thrombolyse beitragen und ihren Nutzen insgesamt weiter steigern. Daher war es Ziel dieser Studie, ein Schlaganfallmodell mit tPA-assoziierter HT in Mäusen unter DAPT zu etablieren, um damit erste Bewertungen therapeutischer Ansätze zur Begrenzung der HT zu ermöglichen.
Ein entscheidender Aspekt vorab war die Bestimmung der Thrombozytenfunktion in den behandelten Mäusen, um damit die Wirksamkeit der DAPT zu messen. Dies war besonders vor dem Hintergrund wichtig, dass DAPT bei Patienten unterschiedlich wirksam ist. So gibt es einen gewissen Anteil Patienten, der resistent gegenüber Aspirin und/oder anderen Thrombozytenaggregationshemmern wie Clopidogrel zu sein scheint. Daher galt es, dieses Phänomen in unserem Modell zu kontrollieren und etwaige Non-Responder zu identifizieren und gegebenenfalls auszuschließen. Dies ist bei herkömmlichen Methoden der Aggregometrie (dem Standardverfahren zur Messung der Thrombozytenfunktion und Therapieüberwachung von Thrombozytenaggregationshemmern) eine Herausforderung, da im Handel erhältliche Aggregometer Blutvolumina erfordern, die für eine Maus tödlich wären. Auch Schwanzblutungstests (sog. „tail bleeding tests“) versagen häufig, wenn sie nach einer experimentellen Schlaganfalloperation durchgeführt werden. Wir haben daher einen Durchflusszytometrie-basierten Ansatz zur Messung der in vitro Thrombozytenfunktion modifiziert, der nur geringe Blutvolumina erfordert und von uns erstmals in einem experimentellen Schlaganfallprotokoll eingesetzt wurde. Dieser zeigte eine signifikant reduzierte Thrombozytenfunktion nach DAPT mit Aspirin und Clopidogrel (ASA+CPG) an. Die Methode korrelierte gut mit Ergebnissen von zusätzlich durchgeführten Schwanzblutungstests und wird künftige präklinische Studien zur DAPT in Mäusen erleichtern. Obwohl es eine gewisse Variabilität in der Thrombozytenfunktion der behandelten Mäuse gab, identifizierten wir letztendlich keine Non-Responder.
Als nächstes zeigten wir erfolgreich, dass DAPT mit ASA+CPG in Mäusen beim experimentellen Schlaganfall zu vermehrter HT beiträgt. Wurde die DAPT mit einer tPA-Thrombolyse verbunden, erhöhte sich die HT-Rate sogar signifikant im Vergleich zu unbehandelten Mäusen mit und ohne tPA-Thrombolyse. Unser Modell kann nun genutzt werden, um die Mechanismen der HT weiter zu untersuchen. Noch wichtiger ist, dass die Einrichtung eines solchen Modells es Forschern ermöglicht, mögliche Strategien zur Minderung des Blutungsrisikos bei Patienten mit DAPT zu testen.
Zur Verringerung der HT wählten wir zwei verschiedene pharmakologische Strategien. Zunächst untersuchten wir die Reduktion der tPA Dosis, welche allerdings nicht erfolgreich vor hämorrhagischen Komplikationen schützen konnte. Danach fokussierten wir uns auf die Rolle der 12/15-Lipoxygenase (12/15-LOX) in unserem Modell. Verschiedene Vorarbeiten hatten gezeigt, dass die 12/15-LOX zum Untergang von Endothelzellen im ischämischen Gehirn beiträgt und damit wahrscheinlich eine ursächliche oder zumindest unterstützende Rolle in der Entstehung der HT hat. So wiederholten wir unsere Versuche der tPA-assoziierten HT unter DAPT in LOX-knockout Mäusen und inhibierten die 12/15-LOX pharmakologisch mit ML351. Wir zeigten erfolgreich, dass die Hemmung von 12/15-LOX in Wildtyp-Mäusen die Blutungsrate signifikant reduzierte und identifizierten die 12/15-LOX damit als geeigneten Kandidaten für weiterführende Studien zur Eindämmung sekundärer Schäden nach ischämischen Schlaganfall. Zudem wäre neben der therapeutischen, auch die prophylaktische Gabe von 12/15-LOX Inhibitoren in Hochrisikopatienten additiv zur Thrombolyse denkbar. Eine solche Blutungsprophylaxe könnte zu einer Indikationserweiterung der Lysetherapie beitragen und das funktionelle Langzeit-Ergebnis der Patienten verbessern.
Therapy resistance in leukemia may be due to cancer cell-intrinsic and/or -extrinsic mechanisms. Mutations within BCR-ABL1, the oncogene giving rise to chronic myeloid leukemia (CML), lead to resistance to tyrosine kinase inhibitors (TKI), and some are associated with clinically more aggressive disease and worse outcome. Using the retroviral transduction/transplantation model of CML and human cell lines we faithfully recapitulate accelerated disease course in TKI resistance. We show in various models, that murine and human imatinib-resistant leukemia cells positive for the oncogene BCR-ABL1T315I differ from BCR-ABL1 native (BCR-ABL1) cells with regards to niche location and specific niche interactions. We implicate a pathway via integrin β3, integrin-linked kinase (ILK) and its role in deposition of the extracellular matrix (ECM) protein fibronectin as causative of these differences. We demonstrate a trend towards a reduced BCR-ABL1T315I+ tumor burden and significantly prolonged survival of mice with BCR-ABL1T315I+ CML treated with fibronectin or an ILK inhibitor in xenogeneic and syngeneic murine transplantation models, respectively. These data suggest that interactions with ECM proteins via the integrin β3/ILK-mediated signaling pathway in BCR-ABL1T315I+ cells differentially and specifically influence leukemia progression. Niche targeting via modulation of the ECM may be a feasible therapeutic approach to consider in this setting.
Background: Previously, we used inhibitors blocking BET bromodomain binding proteins (BRDs) in Ewing sarcoma (EwS) and observed that long term treatment resulted in the development of resistance. Here, we analyze the possible interaction of BRD4 with cyclin-dependent kinase (CDK) 9. Methods: Co-immunoprecipitation experiments (CoIP) to characterize BRD4 interaction and functional consequences of inhibiting transcriptional elongation were assessed using drugs targeting of BRD4 or CDK9, either alone or in combination. Results: CoIP revealed an interaction of BRD4 with EWS-FLI1 and CDK9 in EwS. Treatment of EwS cells with CDKI-73, a specific CDK9 inhibitor (CDK9i), induced a rapid downregulation of EWS-FLI1 expression and block of contact-dependent growth. CDKI-73 induced apoptosis in EwS, as depicted by cleavage of Caspase 7 (CASP7), PARP and increased CASP3 activity, similar to JQ1. Microarray analysis following CDKI-73 treatment uncovered a transcriptional program that was only partially comparable to BRD inhibition. Strikingly, combined treatment of EwS with BRD- and CDK9-inhibitors re-sensitized cells, and was overall more effective than individual drugs not only in vitro but also in a preclinical mouse model in vivo. Conclusion: Treatment with BRD inhibitors in combination with CDK9i offers a new treatment option that significantly blocks the pathognomonic EWS-ETS transcriptional program and malignant phenotype of EwS.
Objective: To evaluate prognostic factors in pediatric patients with gonadal germ cell tumors (GCT). Methods: Patients <18 years with ovarian and testicular GCT (respectively OGCT and TGCT) were prospectively registered according to the guidelines of MAKEI 96. After resection of the primary tumor, patients staged ≥II received risk-stratified cisplatin-based combination chemotherapy. Patients were analyzed in respect to age (six age groups divided into 3-year intervals), histology, stage, and therapy. The primary end point was overall survival. Results: Between January 1996 and March 2016, the following patients were registered: 1047 OGCT, of those, 630 had ovarian teratoma (OTER) and 417 had malignant OGCT (MOGCT); and 418 TGCT, of those, 106 had testicular teratoma (TTER) and 312 had malignant TGCT (MTGCT). Only in MTGCT, older age correlated with a higher proportion of advanced tumors. All 736 teratomas and 240/415 stage I malignant gonadal GCT underwent surgery and close observation alone. In case of watchful waiting, the progression rate of OGCT was higher than that of TGCT. However, death from disease was reported in 8/417 (1.9%) MOGCT and 8/312 (2.6%) MTGCT irrespective of adjuvant chemotherapy and repeated surgery. Conclusions: The different pathogenesis and histogenesis of gonadal GCT reflects sex- and age-specific patterns that define clinically relevant risk groups. Therefore, gender and age should be considered in further research on the biology and clinical practice of pediatric gonadal GCT.
Despite the success of immune checkpoint blockade in cancer, the number of patients that benefit from this revolutionary treatment option remains low. Therefore, efforts are being undertaken to sensitize tumors for immune checkpoint blockade, which includes combining immune checkpoint blocking agents such as anti-PD-1 antibodies with standard of care treatments. Here we report that a combination of chemotherapy (doxorubicin) and immune checkpoint blockade (anti-PD-1 antibodies) induces superior tumor control compared to chemotherapy and immune checkpoint blockade alone in the murine autochthonous polyoma middle T oncogene-driven (PyMT) mammary tumor model. Using whole transcriptome analysis, we identified a set of genes that were upregulated specifically upon chemoimmunotherapy. This gene signature and, more specifically, a condensed four-gene signature predicted favorable survival of human mammary carcinoma patients in the METABRIC cohort. Moreover, PyMT tumors treated with chemoimmunotherapy contained higher levels of cytotoxic lymphocytes, particularly natural killer cells (NK cells). Gene set enrichment analysis and bead-based ELISA measurements revealed increased IL-27 production and signaling in PyMT tumors upon chemoimmunotherapy. Moreover, IL-27 signaling improved NK cell cytotoxicity against PyMT cells in vitro. Taken together, our data support recent clinical observations indicating a benefit of chemoimmunotherapy compared to monotherapy in breast cancer and suggest potential underlying mechanisms.
