Für Patienten mit lebensbedrohlichen Herzkrankheiten ist die Extracorporeal-Life-Support-Behandlung (ECLS) eine sinnvolle Therapiemöglichkeit. Sie bietet für Patienten im kardiogenen Schock ein Zeitfenster, um eine myokardiale Erholung zu erreichen. Hierbei kann in Abhängigkeit vom Krankheitsbild die zusätzliche Anwendung von IABP die Heilungschancen begünstigen. In der durchgeführten retrospektiven Studie wurden 118 Patienten betrachtet, die in der Klinik für Thorax-, Herz- und thorakale Gefäßchirurgie der Universität Frankfurt am Main im Zeitraum von Dezember 2001 bis Ende 2013 eine ECLS-Therapie erhalten haben. Bei 59 Patienten wurde die ECLS-Unterstützung in Kombination mit IABP durchgeführt. Die beiden Patientenkollektive - mit und
ohne IABP- sind hinsichtlich ihrer Risikofaktoren vergleichbar. Ausgehend von der Zielsetzung dieser Arbeit wurde analysiert, ob der gleichzeitige Einsatz der IABP bei ECLS – Therapie von Vor- oder sogar von Nachteil ist. Hierfür wurden für die beiden Therapiegruppen Überlebenszeitanalysen nach Kaplan-Meier durchgeführt. Der statistische Vergleich der Überlebensraten und des Weaningerfolgs erfolgte mit Hilfe des Log-Rank-Tests.
Die Auswertung der erhobenen Daten hat ergeben, dass kein signifikanter Unterschied bei der 30-Tages-Überlebensrate und dem Weaningerfolg für die beiden Patientenkollektive mit und ohne zusätzliche IABP-Anwendung vorhanden ist. Risikofaktoren wie ein hohes Lebensalter oder eine bereits vor Einlieferung stattgefundene Intubation verringern außerdem die Überlebenschancen nach ECLS - Therapie. Ein fortgeschrittenes NYHA-Stadium konnte nicht als negativer prädiktiver Faktor identifiziert werden.
In der Literatur kommt man bezüglich der Mortalität unter alleiniger ECLS-Therapie oder dem zusätzlichen Einsatz einer IABP zu unterschiedlichen Ergebnissen. Diese besagen teilweise, dass ECLS und IABP einander ergänzende Methoden sind, die sich durchaus synergistisch auf den Behandlungserfolg auswirken können und dass die Mortalität beim zusätzlichen Einsatz einer IABP signifikant niedriger ist. Die Durchführung weiterer prospektiver Studien mit vergleichbaren Patientenkollektiven zur Untersuchung des Outcomes bei den unterschiedlichen Behandlungsmethoden ist jedoch erforderlich, um ein aussagekräftiges Fazit ziehen zu können.
Background: Unwanted anticholinergic effects are both underestimated and frequently overlooked. Failure to identify adverse drug reactions (ADRs) can lead to prescribing cascades and the unnecessary use of over-thecounter products. The objective of this systematic review and meta-analysis is to explore and quantify the frequency and severity of ADRs associated with amitriptyline vs. placebo in randomized controlled trials (RCTs) involving adults with any indication, as well as healthy individuals. Methods: A systematic search in six electronic databases, forward/backward searches, manual searches, and searches for Food and Drug Administration (FDA) and European Medicines Agency (EMA) approval studies, will be performed. Placebo-controlled RCTs evaluating amitriptyline in any dosage, regardless of indication and without restrictions on the time and language of publication, will be included, as will healthy individuals. Studies of topical amitriptyline, combination therapies, or including <100 participants, will be excluded. Two investigators will screen the studies independently, assess methodological quality, and extract data on design, population, intervention, and outcomes ((non-)anticholinergic ADRs, e.g., symptoms, test results, and adverse drug events (ADEs) such as falls). The primary outcome will be the frequency of anticholinergic ADRs as a binary outcome (absolute number of patients with/without anticholinergic ADRs) in amitriptyline vs. placebo groups. Anticholinergic ADRs will be defined by an experienced clinical pharmacologist, based on literature and data from Martindale: The Complete Drug Reference. Secondary outcomes will be frequency and severity of (non-)anticholinergic ADRs and ADEs. The information will be synthesized in meta-analyses and narratives. We intend to assess heterogeneity using metaregression (for indication, outcome, and time points) and I2 statistics. Binary outcomes will be expressed as odds ratios, and continuous outcomes as standardized mean differences. Effect measures will be provided using 95% confidence intervals. We plan sensitivity analyses to assess methodological quality, outcome reporting etc., and subgroup analyses on age, dosage, and duration of treatment. Discussion: We will quantify the frequency of anticholinergic and other ADRs/ADEs in adults taking amitriptyline for any indication by comparing rates for amitriptyline vs. placebo, hence, preventing bias from disease symptoms and nocebo effects. As no standardized instrument exists to measure it, our overall estimate of anticholinergic ADRs may have limitations.
Background: Current literature is inconsistent regarding the risk of severe side effects using accelerated induction protocols in Hymenoptera venom immunotherapy (VIT). In addition, several data indicate the influence of purity grade of venom preparation on tolerability. We evaluated the safety and tolerability of ultra-rush and rush build-up protocols using purified and non-purified venom preparations. Methods: Retrospective single-center study of 581 VIT inductions (325 ultra-rush and 256 rush protocols) from 2005 to 2018 in 559 patients with bee and vespid venom allergy using aqueous purified (ALK SQ®) for ultra-rush protocol and aqueous non-purified (ALK Reless®) venom preparations for rush protocol. Results: Urticaria (8% vs. 3.1%, p = 0,013) and dose reductions (4.3% vs. 1.2%, p = 0,026) were significantly more frequent in the ultra-rush group. Overall rate of moderate-to-severe side effects (anaphylaxis ≥grade 2 according to Ring and Meβmer) was low and did not differ significantly between protocols (p = 0.105). Severe events (grade 4 anaphylaxis) were not reported. Discontinuation rate was very low in both cohorts (0.6% vs 1.2%). The higher purity grade of venom preparations in the ultra-rush cohort did not improve tolerability. The bee venom group showed a non-significant trend towards higher incidence of mild reactions (urticaria), resulting in more frequent dose reductions and antiallergic therapy. Conclusion: Rush and ultra-rush protocols show an excellent safety profile with only infrequent and mild anaphylactic reactions in bee and vespid venom allergy. Ultra-rush immunotherapy reduces the duration of the inpatient build-up phase setting and thus is viewed by the authors as preferred treatment in Hymenoptera venom allergic patients.
Excavation of burrows is an extremely physically demanding activity producing a large amount of metabolic heat. Dissipation of its surplus is crucial to avoid the risk of overheating, but in subterranean mammals it is complicated due to the absence of notable body extremities and high humidity in their burrows. IR-thermography in a previous study on two species of African mole-rats revealed that body heat was dissipated mainly through the ventral body part, which is notably less furred. Here, we analyzed the dorsal and ventral skin morphology, to test if dermal characteristics could contribute to higher heat dissipation through the ventral body part. The thickness of the epidermis and dermis and the presence, extent and connectivity of fat tissue in the dermis were examined using routine histological methods, while vascular density was evaluated using fluorescent dye and confocal microscopy in the giant mole-rat Fukomys mechowii. As in other hitherto studied subterranean mammals, no subcutaneous adipose tissue was found. All examined skin characteristics were very similar for both dorsal and ventral regions: relative content of adipose tissue in the dermis (14.4 ± 3.7% dorsally and 11.0 ± 4.0% ventrally), connectivity of dermal fat (98.5 ± 2.8% and 95.5 ± 6.8%), vascular density (26.5 ± 3.3% and 22.7 ± 2.3%). Absence of large differences in measured characteristics between particular body regions indicates that the thermal windows are determined mainly by the pelage characteristics.
Effective connectivity (EC) is able to explore causal effects between brain areas and can depict mechanisms that underlie repair and adaptation in chronic brain diseases. Thus, the application of EC techniques in multiple sclerosis (MS) has the potential to determine directionality of neuronal interactions and may provide an imaging biomarker for disease progression. Here, serial longitudinal structural and resting-state fMRI was performed at 12-week intervals over one year in twelve MS patients. Twelve healthy subjects served as controls (HC). Two approaches for EC quantification were used: Causal Bayesian Network (CBN) and Time-resolved Partial Directed Coherence (TPDC). The EC strength was correlated with the Expanded Disability Status Scale (EDSS) and Fatigue Scale for Motor and Cognitive functions (FSMC). Our findings demonstrated a longitudinal increase in EC between specific brain regions, detected in both the CBN and TPDC analysis in MS patients. In particular, EC from the deep grey matter, frontal, prefrontal and temporal regions showed a continuous increase over the study period. No longitudinal changes in EC were attested in HC during the study. Furthermore, we observed an association between clinical performance and EC strength. In particular, the EC increase in fronto-cerebellar connections showed an inverse correlation with the EDSS and FSMC. Our data depict continuous functional reorganization between specific brain regions indicated by increasing EC over time in MS, which is not detectable in HC. In particular, fronto-cerebellar connections, which were closely related to clinical performance, may provide a marker of brain plasticity and functional reserve in MS.
Background: The vascular effects of training under blood flow restriction (BFR) in healthy persons can serve as a model for the exercise mechanism in lower extremity arterial disease (LEAD) patients. Both mechanisms are, inter alia, characterized by lower blood flow in the lower limbs. We aimed to describe and compare the underlying mechanism of exercise-induced effects of disease- and external application-BFR methods. Methods: We completed a narrative focus review after systematic literature research. We included only studies on healthy participants or those with LEAD. Both male and female adults were considered eligible. The target intervention was exercise with a reduced blood flow due to disease or external application. Results: We identified 416 publications. After the application of inclusion and exclusion criteria, 39 manuscripts were included in the vascular adaption part. Major mechanisms involving exercise-mediated benefits in treating LEAD included: inflammatory processes suppression, proinflammatory immune cells, improvement of endothelial function, remodeling of skeletal muscle, and additional vascularization (arteriogenesis). Mechanisms resulting from external BFR application included: increased release of anabolic growth factors, stimulated muscle protein synthesis, higher concentrations of heat shock proteins and nitric oxide synthase, lower levels in myostatin, and stimulation of S6K1. Conclusions: A main difference between the two comparators is the venous blood return, which is restricted in BFR but not in LEAD. Major similarities include the overall ischemic situation, the changes in microRNA (miRNA) expression, and the increased production of NOS with their associated arteriogenesis after training with BFR.
The macrophage-inducible C-type lectin (mincle) is part of the innate immune system and acts as a pattern recognition receptor for pathogen-associated molecular patterns (PAMPS) and damage-associated molecular patterns (DAMPs). Ligand binding induces mincle activation which consequently interacts with the signaling adapter Fc receptor, SYK, and NF-kappa-B. There is also evidence that mincle expressed on macrophages promotes intestinal barrier integrity. However, little is known about the role of mincle in hepatic fibrosis, especially in more advanced disease stages. Mincle expression was measured in human liver samples from cirrhotic patients and donors collected at liver transplantation and in patients undergoing bariatric surgery. Human results were confirmed in rodent models of cirrhosis and acute-on-chronic liver failure (ACLF). In these models, the role of mincle was investigated in liver samples as well as in peripheral blood monocytes (PBMC), tissues from the kidney, spleen, small intestine, and heart. Additionally, mincle activation was stimulated in experimental non-alcoholic steatohepatitis (NASH) by treatment with mincle agonist trehalose-6,6-dibehenate (TDB). In human NASH, mincle is upregulated with increased collagen production. In ApoE deficient mice fed high-fat western diet (NASH model), mincle activation significantly increases hepatic collagen production. In human cirrhosis, mincle expression is also significantly upregulated. Furthermore, mincle expression is associated with the stage of chronic liver disease. This could be confirmed in rat models of cirrhosis and ACLF. ACLF was induced by LPS injection in cirrhotic rats. While mincle expression and downstream signaling via FC receptor gamma, SYK, and NF-kappa-B are upregulated in the liver, they are downregulated in PBMCs of these rats. Although mincle expressed on macrophages might be beneficial for intestinal barrier integrity, it seems to contribute to inflammation and fibrosis once the intestinal barrier becomes leaky in advanced stages of chronic liver disease.
Einleitung: OTSC Proctology ist ein Verfahren in der Analfistelchirurgie dessen Erfolgsaussichten auch 9 Jahre nach der ersten klinischen Anwendung nicht abschließend beurteilt werden können. Die wenigen bisher publizierten Studien zeigen sehr divergente Ergebnisse mit Heilungsraten von 10 bis 90%.
Material und Methoden: Wir führten eine retrospektive Auswertung der Behandlungsergebnisse aller konsekutiven Patienten, die in dem Zeitraum vom 01.03.2014 bis 31.03.2017 in der koloproktologischen Abteilung der DKD Helios Klinik Wiesbaden mittels OTSC-Verfahren wegen Analfisteln operiert wurden, durch. Erfasst wurden Alter, Geschlecht, OP- und Aufenthaltsdauer, Operateur, Fistelart und –lokalisation, Vorhandensein von Stoma und CED, plastischer Fistelverschluss (PFV) in der Anamnese, Clipverbleib nach der OP, Dauer des Follow-ups, Komplikationen sowie postoperative Schmerzsituation. Die Datenauswertung erfolgte mittels deskriptiver Statistik bei Subgruppenanalyse unter Verwendung der Statistiksoftware SPSS 20.
Ergebnisse: Es wurden insgesamt 68 Fälle eingeschlossen, davon 37% weiblich und 63% männlich. Das durchschnittliche Alter betrug 52 Jahre (25 – 81). 19 (28%) Patienten litten an CED, 11 (16%) Patienten hatten ein Stoma. 34 (50%) der Patienten hatten plastischen Fistelverschluss in der Anamnese. Die Verteilung nach Fisteltyp war wie folgt: 58 (85%) transsphinktär, 4 (6%) suprasphinktär, 3 (4%) intersphinktär, 1 (1,5%) rektovaginal, 1 (1,5%) rektourethral und 1 (1,5%) Pouchfistel. Die häufigsten Fistelokalisationen waren bei 6 h (N = 26, 38%), 12 h (N = 14, 21%), 7 h (N = 7, 10%) und 3h (N = 5, 7%) SSL. Die durchschnittlichen OP-Dauer und stationärer Aufenthalt betrugen 25 min (6 – 90) und 7 Tage (1 – 14 Tage) entsprechend. Die durchschnittliche Dauer des Follow-ups betrug 29 Monate (10 – 36).
Die Fistelheilung im Gesamtkollektiv lag bei 48,5%, 1 (1,5%) Patient war lost-tofollow-up. In der weiblichen (N = 25) und männlichen (N = 43) Kohorte fand die Heilung in 40% und 53% der Fälle entsprechend statt. Die Heilungsraten bei intersphinktären (N = 3), transsphinktären (N = 58) und suprasphinktären (N = 4) Fisteln lagen bei 100%, 46,5% und 50% entsprechend, eine rektovaginale und eine rektourethrale Fistel sind nicht geheilt. Eine Pouchfistel ist geheilt.
Die Heilungsraten bei 6h und 12h SSL lagen bei 58% und 14% entsprechend mit deutlichem Vorteil bei posteriorer Fistellage. Dieser Vorteil blieb nach der Aufteilung der Fisteln in anteriore (N = 30) und posteriore (N = 38), mit
Heilungsraten von 33% und 60,5% entsprechend, bestehen. In den Subgruppen ohne CED (N = 49) und mit CED (N = 19) lagen die Heilungsraten bei 53% und 35% entsprechend. In den Subgruppen ohne PFV (N = 34) und mit PFV (N = 34) lagen die Heilungsraten bei 59% und 38% entsprechend. In der Subgruppe ohne Stoma (N = 57) wurde eine Heilung in 47% der Fälle, in der Subgruppe mit Stoma (N = 11) in 55% beobachtet. In der Subgruppe mit den kryploglandulären Fisteln (N = 47) war die Heilung in 55,3% zu sehen und in der Subgruppe mit
kryptoglandulären Fisteln ohne PFV (N = 27) bei 63%.
In 48 (70,6%) Fällen wurde der Clip aktiv entfernt, in 11 (16,2%) Fällen kam es zum Spontanverlust und in 8 (11,8%) Fällen blieb der Clip in situ. Die durchschnittliche Zeit bis zur Klammerentfernung betrug 4 Monate. Die Heilungsraten bei Clipentfernung, Clipverbleib und Clipspontanabgang lagen bei 42%, 100% und 45% entsprechend.
Der maximale Schmerz nach NRS 0 – 2 bei 61% der Patienten, NRS 3 – 4 bei 28% und NRS 5 – 7 nur bei 11%. Bei 50% der Fälle war kein Opiat erforderlich und bei 39% der Fälle erfolgte die Opiateinnahme nicht länger als 2 Tage.
Die Komplikationen waren sehr selten: eine Nachblutung mit Clipdislokation (1,5%), ein Analabszess (1,5%), 2 Fälle (3%) der neuaufgetretenen Stuhlschmieren und 1 (1,5%) Wundheilungsstörung intraanal, die spontan abheilte. In 4 (6%) Fällen kam es zur Klammerdislokation vom inneren
Fistelostium mit konsekutiver Fistelpersistenz.
Fazit: OTSC ist ein komplikationsarmes und schmerzarmes Verfahren mit kurzer OP-Zeit und könnte einen festen Platz in der Analfistelchirurgie einnehmen. Die bestmöglichen Ergebnisse lassen sich bei dorsal gelegenen Analfisteln in
nichtvoroperierten Patienten ohne CED erzielen.
Aims: SARS-CoV-2 infection is associated with adverse outcomes in patients with cardiovascular disease. Here, we analyzed whether specific biomarkers predict the clinical course of COVID-19 in patients with cardiovascular comorbidities. Methods and results: We enrolled 2147 patients with SARS-CoV-2 infection which were included in the Lean European Open Survey on SARS-CoV‑2 (LEOSS)-registry from March to June 2020. Clinical data and laboratory values were collected and compared between patients with and without cardiovascular comorbidities in different clinical stages of the disease. Predictors for mortality were calculated using multivariate regression analysis. We show that patients with cardiovascular comorbidities display significantly higher markers of myocardial injury and thrombo-inflammatory activation already in the uncomplicated phase of COVID-19. In multivariate analysis, elevated levels of troponin [OR 1.54; (95% CI 1.22–1.96), p < 0.001)], IL-6 [OR 1.69 (95% CI 1.26–2.27), p < 0.013)], and CRP [OR 1.32; (95% CI 1.1–1.58), p < 0.003)] were predictors of mortality in patients with COVID-19. Conclusion: Patients with cardiovascular comorbidities show elevated markers of thrombo-inflammatory activation and myocardial injury, which predict mortality, already in the uncomplicated phase of COVID-19. Starting targeted anti-inflammatory therapy and aggressive anticoagulation already in the uncomplicated phase of the disease might improve outcomes after SARS-CoV-2 infection in patients with cardiovascular comorbidities.
Background: The ERGO2 (Ernaehrungsumstellung bei Patienten mit Rezidiv eines Glioblastoms) MR-spectroscopic imaging (MRSI) subtrial investigated metabolism in patients randomized to calorically restricted ketogenic diet/intermittent fasting (crKD-IF) versus standard diet (SD) in addition to re-irradiation (RT) for recurrent malignant glioma. Intracerebral concentrations of ketone bodies (KB), intracellular pH (pHi), and adenosine triphosphate (ATP) were non-invasively determined. Methods: 50 patients were randomized (1:1): Group A keeping a crKD-IF for nine days, and Group B a SD. RT was performed on day 4-8. Twenty-three patients received an extended MRSI-protocol (1H decoupled 31P MRSI with 3D chemical shift imaging (CSI) and 2D 1H point-resolved spectroscopy (PRESS)) at a 3T scanner at baseline and on day 6. Voxels were selected from the area of recurrent tumor and contralateral hemisphere. Spectra were analyzed with LCModel, adding simulated signals of 3-hydroxybutyrate (βOHB), acetone (Acn) and acetoacetate (AcAc) to the standard basis set. Results: Acn was the only reliably MRSI-detectable KB within tumor tissue and/or normal appearing white matter (NAWM). It was detected in 4/11 patients in Group A and in 0/8 patients in Group B. MRSI results showed no significant depletion of ATP in tumor tissue of patients at day 6 during crKD-IF, even though there were a significant difference in ketone serum levels between Group A and B at day 6 and a decline in fasting glucose in Group A from baseline to day 6. The tumor specific alkaline pHi was maintained. Conclusions: Our metabolic findings suggest that tumor cells maintain energy homeostasis even with reduced serum glucose levels and may generate additional ATP through other sources.r sources.
