Metastatic rhabdomyosarcoma (RMS) is one of the most challenging tumor entities in pediatric oncology caused by treatment resistances and immune escape. Novel chimeric antigen receptor (CAR) immunotherapies as specific, effective and safe treatment provide antitumor cytotoxicity by soluble factors and ligands/receptor signals. Besides its intrinsic potential as innate immune cell the ErbB2-sprecific CAR-engineered natural killer (NK)-92 cell line NK-92/5.28.z also provides CAR-mediated cytotoxicity, resulting in a high lytic capacity against 2D and 3D RMS cell structures in vitro. Also in a xenograft model using immune deficient NOD/Scid/IL2Rγ-/- (NSG) mice inhibited NK-92/5.28.z the tumor growth as long as the cells were administered and therefore prolonged the survival of the animals. The NK-92/5.28.z were distributed by the blood circulation and subsequently infiltrated the tumor tissue. Due to the malignant origin of the NK-92 cell line the cells must be irradiated prior to the use in patients. While the irradiation hampered the proliferation of NK-92/5.28.z cells, the cytotoxicity against RMS cells in vitro is retained for at least 24 hours. In the xenograft model irradiated NK-92/5.28.z cells inhibited the tumor growth but to a lower extent than untreated cells, as irradiated cells have only a limited life span in vivo no durable persistence and remission was achieved. Therefore, combinatorial approaches were focused and while blocking of the PD-1/PD-L1 axis did not resulted in a significantly enhanced tumor cell lysis, the combinatorial treatment with proteasome inhibitor bortezomib exhibited a significant enhanced cytotoxicity against RMS cells at least in vitro. Bortezomib itself induces caspase mediated apoptosis and also the upregulates the expression of TRAIL receptor DR5. The corresponding ligand TRAIL is expressed on the surface of the NK-92/5.28.z and pursuing experiments with purified TRAIL and bortezomib revealed a synergism. NK-92/5.28.z as an off-the-shelf product is therefore feasible for the therapy of metastatic RMS, but it might be necessary to support the cytotoxicity by additive agents like proteasome inhibitor bortezomib to archive durable remission.
Another cell population suitable for RMS CAR-immunotherapy are cytokine induced killer (CIK) cells, a heterogenous cell population generated from autologous PBMCs consisting of T, NK and T-NK cells. Lentivirally transduced ErbB2-specific CAR-CIK cells were previously shown to inhibit the tumor engraftment in a RMS xenograft model. However, lentiviral transduced adoptive immunotherapies bear risks for the transfer in patients, therefore the Sleeping Beauty Transposon System (SBTS) as a non-viral method, which integrates the CAR coding DNA by a cut-and-paste mechanism from a minicircle (MC) into the CIK cells genome is more feasible for the generation of CAR-CIK cells. The Sleeping beauty transposase mRNA and the MC were transferred in the cell by nucleofection, different factors influence the transfection efficiency and viability of the CIK cells in this harsh procedure. In preliminary experiments with MC Venus, a MC encoding eGFP, the highest transfection efficiency with the best proliferative capacity was achieved with cells on day 3 of CIK culture and without the addition of autologous monocytes as feeder cells. For the CAR construct the protocol was further improved by adjusting crucial factors, for this construct the best results were achieved on day 0, without irradiated PBMCs as feeder cells and cultivation in X-Vivo10 medium supplemented with human fresh frozen plasma. The X-Vivo10 medium enhanced the percentage of NK- and T-NK cells significantly compared to CAR-CIK cells cultured in RPMI. Since the gene transfer by SBTS resulted in CAR-CIK cells stably expressing a CAR in all subpopulations, resulting in a significantly enhanced cytotoxicity against RMS cells in vitro, these cells were compared to lentiviral transduced CAR-CIK cells in vitro and in vivo. While the SBTS CAR-CIK cells were superior to viral CAR-CIK cells in 2D short-term assays, the viral cells showed higher lytic capacity in 3D spheroid long-term assays. In a RMS xenograft model lentiviral CAR-CIK cells significantly prolonged the survival of mice and persisted, whereas SBTS CAR-CIKs did not favor the overall survival compared to untreated controls and also did not persist. Phenotypic analysis revealed a highly cytotoxic CD8+ and late effector memory dominant phenotype for SBTS CAR-CIK cells supporting short-term cytotoxicity but also more prone for exhaustion, while viral CAR-CIK cells showed a more balanced phenotype for memory and cytotoxicity. Therefore, the SBTS is feasible for the ErbB2-CAR gene transfer in CAR-CIK resulting in a stable CAR-expression with high short-term cytotoxicity, but these cells are also more prone to exhaustion and the protocol might be adapted further to prevent this limitation for in vivo application.
This work underlines the hard-to-treat characteristics of metastatic RMS, but also shows some approaches for further evaluation like the combination of NK-92/5.28.z cells with bortezomib and the feasibility of the generation of CAR-CIK cells via SBTS.
In dieser retrospektiven Studie wurden 47 Patienten mit einem histologisch nachgewiesenen Analkarzinom und im Anschluss aufgetretenen Rezidiv nach Beendigung der Radiochemotherapie und hinsichtlich der Risikofaktoren für das tumorfreie, sowie das Gesamtüberleben ausgewertet.
Auffällig in dieser Patientenkohorte war der hohe Anteil an männlichen Patienten (68,1 %), an HIV-positiven (36,2 %) sowie an Patienten mit nachgewiesenem Lymphknotenbefall bei Erstdiagnose (72,3 %).
Als signifikante Risikofaktoren für das tumorfreie Überleben wurden ein Primärtumor ab ≥ T3-Kategorie (Hazard-Ratio 1,87), ein Karnofsky-Performance-Status ≤ 80 % vor Beginn der Radiochemotherapie (Hazard-Ratio 3,25), sowie das initiale fehlende therapeutische Ansprechen der Radiochemotherapie (Hazard-Ratio 5,9) festgestellt. Auffällig war, dass kein signifikanter Einfluss bzgl. des biologischen Geschlechts, des Gradings, der N Kategorie oder des Alters ermittelt werden konnte.
In Bezug auf das Gesamtüberleben der Patienten ergaben sich folgende signifikante Risikofaktoren: eine T-Kategorie ≥ T3 (Hazard-Ratio 4,091), ein hohes UICC-Stadium (Hazard Ratio 2,89 für Stadium IIIC), das fehlende initiale therapeutische Ansprechen der Radiochemotherapie (Hazard-Ratio 9,59), ein Karnofsky-Performance-Status ≤ 80 % (Hazard-Ratio 12,23). Ein protektiver Faktor stellte ein längeres tumorfreies Überleben (Hazard-Ratio 0,935) dar.
Die Auswertung des gesamten und des tumorfreien Überlebens hinsichtlich des Befallsmusters der Rezidive ergab, dass mit zusätzlich zum lokalen Rezidiv nachgewiesenem lokoregionären Rezidiv und Fernmetastasen sich sowohl das tumorfreie Überleben (Ein-Jahres-tumorfreies Überleben 52,9 ± 12,1 % vs. 15,0 ± 8,0 %) als auch das Gesamtüberleben (5-Jahres-Gesamtüberleben 75,0 ± 12,5 % vs. 0,0 %) signifikant verringerten.
Im Vergleich der Merkmale der Patienten mit und ohne nachgewiesenen Fernmetastasen ergab sich ein signifikanter Unterschied in Bezug auf den Anteil der Tumore ≥ T3 (75 %).
In der Untergruppe, die mittels abdominoperinealer Rektumexstirpation therapiert wurde, konnte kein signifikanter Unterschied in Bezug auf die rpT-Kategorie, R-Klassikfikation, Pn-, V- und L-Klassifikation festgestellt werden.
Background: This study investigates (1) whether alterations in magnetic resonance imaging (MRI)-based structural global network organization is impaired in patients with major depressive disorder (MDD), (2) whether in-patient treatment including pharmacological, psychological and neurostimulation interventions is linked to changes in structural brain connectivity and (3) whether brain structural changes relate to changes in depression symptomatology.
Methods: One hundred seventy-eight subjects – 109 subjects diagnosed with current MDD and 55 healthy controls (HC) - participated in the present study (baseline + 6-weeks follow up). Fifty-six depressed patients were treated with electroconvulsive therapy (ECT) and 67 received in-patient treatment without ECT. Here, grey matter T1-weighted MRI was used to define nodes and DWI-based tractography to define the connections – or edges – between the nodes creating a structural connectome. Changes over time in depressions symptom severity was measured with the Hamilton Depression Ratings Scale.
Results: MDD patients showed reduced connectivity strength at baseline compared to healthy controls. MDD patients showed a significant increase of connectivity strength over time, an effect that was not detected in HC. An increase of connectivity strength was associated with a decrease in depression symptom severity. These effects were independent of treatment choice, suggesting a nonspecific effect that cannot be traced back to ECT.
Conclusion: We demonstrate an alleviation of structural brain dysconnectivity in MDD patients after successful antidepressive treatment, which is most prominent in those patients that show the greatest reduction in depressive symptomatology. This pattern of results suggests neuroplastic mechanisms involved in the successful treatment of depression and should be investigated as a potential treatment target in future studies.

Introduction: The COVID-19 pandemic has necessitated a reduction in face-to-face consultations, resulting in significant limitations in healthcare for individuals with depression. To ensure safe and adequate care, e-health services, such as telemedicine, gained a more prominent role. Governments have eased restrictions on the use of telemedicine, enabling healthcare professionals to increasingly offer video and telephone consultations.
Objective: This study examines, 1) possible changes over the course of the pandemic in reported use of video and telephone consultations and intended future use of video consultations with healthcare professionals among adults with diagnosed depression; 2) their attitudes towards video and telephone consultations and perceived barriers towards using e-health after prolonged time of the pandemic; and 3) differences in results between subgroups based on sociodemographic and clinical characteristics.
Methods: Three population-representative online surveys were conducted in Germany at different timepoints (t) during the COVID-19 pandemic. Respondents aged 18–69 years with a professionally diagnosed depression were included in the present analyses (t1: June/July 2020 with n = 1094; t2: February 2021 with n = 1038; t3: September 2021 with n = 1255).
Results: The overall proportion of adults with depression who used video or telephone consultations did not change significantly in the time surveyed (t1: 16.51 %, n = 179; t2: 20.23 %, n = 210; t3: 18.47 %, n = 230). However, among users, reported use of video consultations with a psychotherapist increased significantly from t1 (34.83 %, n = 62) to t3 (44.98 %, n = 102, p = .023). Intended future use of VC for healthcare varied depending on the purpose of the consultation. Significant differences over time were only found for the purpose of using VC to discuss clinical findings, laboratory results and diagnostic analyses with a doctor, with higher intentions reported at t2 during lockdown in Germany. At t3, the majority of adults with depression felt that video and telephone consultations were too impersonal and considered them more as a helpful support rather than an alternative to face-to-face psychotherapy. Key barriers to using e-health were found within the societal context and the lacking support from significant others for using e-health, while knowledge and skills represented facilitators for using e-health.
Conclusion: Despite ambivalent attitudes towards video and telephone consultations among adults with depression, reported use of video consultations with a psychotherapist increased during the COVID-19 pandemic.
We recently described that monoacylglycerol lipase (MGL) is present in the tumor microenvironment (TME), increasing tumor growth. In this study we compare the implications of MGL deficiency in the TME in different tumor types.
We show that subcutaneous injection of KP (KrasLSL-G12D/p53fl/fl, mouse lung adenocarcinoma) or B16-F10 cells (mouse melanoma) induced tumor growth in MGL wild type (WT) and knockout (KO) mice. MGL deficiency in the TME attenuated the growth of KP cell tumors whereas tumors from B16-F10 cells increased in size. Opposite immune cell profiles were detected between the two tumor types in MGL KO mice. In line with their anti-tumorigenic function, the number of CD8+ effector T cells and eosinophils increased in KP cell tumors of MGL KO vs. WT mice whereas their presence was reduced in B16-F10 cell tumors of MGL KO mice. Differences were seen in lipid profiles between the investigated tumor types. 2-arachidonoylglycerol (2-AG) content significantly increased in KP, but not B16-F10 cell tumors of MGL KO vs. WT mice while other endocannabinoid-related lipids remained unchanged. However, profiles of phospho- and lysophospholipids, sphingomyelins and fatty acids in KP cell tumors were clearly distinct to those measured in B16-F10 cell tumors.
Our data indicate that TME-localized MGL impacts tumor growth, as well as levels of 2-AG and other lipids in a tumor specific manner.
Background: Hepatectomy is a complex procedure with high morbidity and mortality.
Early prediction/prevention of major complications is highly valuable for patient care.
Surgical APGAR score (SAS) has been validated to predict post-surgical complications (PCs).
Goal: We aimed to define a simple complications classification following hepatectomy based on a therapy-oriented severity Clavien-Dindo classification (CDC).
Methods: 119 patients undergoing liver resection were included. PCs were determined at follow-up based on CDC. Clinicopathological factors were used to calculate SAS. A receiver-operator characteristic (ROC) curve analysis estimated the predictive value of SAS for PCs. Circulating markers levels of liver injury were analyzed as critical elements on PCs.
Results: SAS (P=0.008), estimated blood-loss (P=0.018) and operation time (P=0.0008) were associated with PCs. SAS was reduced in patients with (+) compared to those without (-) complications (6.64±1.84 vs 5.70±1.79, P=0.0079). The area-under-the-curve was 0.646 by ROC, indicating an acceptable discrimination with 65% possibility to distinguish (-) and (+) groups (P=0.004).
Best cutoff value for SAS was ≤6/≥7, at which sensitivity and specificity were maximal.
ALT/ASL levels were significantly different within the group with 9-10 SAS points (P=0.01 and 0.02).
Conclusion: SAS provides accurate risk stratification for major PCs after hepatectomy, and might help improving the overall patient outcome.
Aktivierende Mutationen der Fms-like tyrosine kinase (FLT3) treten bei 25 % der Patienten mit akuter myeloischer Leukämie (AML) auf und begünstigen die unkontrollierte Proliferation maligner Blasten. Autophagie ist ein intrazellulärer Prozess, durch den zytoplasmatische Bestandteile lysosomal abgebaut werden und fungiert als intrazellulärer Homöostase-Mechanismus unter Stress-Bedingungen.
Ziel dieser Arbeit war es, herauszufinden, ob FLT3-ITD+-AML-Zellen vulnerabel gegenüber Autophagie-Hemmung sind.
Hierzu wurde zunächst untersucht, wie sich FLT3-ITD-Signaling und Autophagie unter basalen Wachstumsbedingungen gegenseitig beeinflussen. In einem genetischen Modell zeigte sich, dass FLT3-ITD-transformierte wachstumsfaktorunabhängige Zellen während ihrer fortgesetzten Proliferation vermehrt Autophagie betreiben. Lysosomale Autophagie-Inhibitoren zeigten jedoch unter diesen Bedingungen keine erhöhte Wirksamkeit gegenüber FLT3-ITD-positiven Zellen. Humane FLT3-ITD-positive AML-Zellen zeigten nach genetischer Deletion von ULK1 ebenfalls nur transiente und milde Proliferationsdefizite. Unter basalen Wachstumsbedingungen zeigte sich also keine erhöhte Vulnerabilität FLT3-ITD-exprimierender Zellen gegenüber Autophagie-Inhibition.
Daraufhin wurde die Bedeutung von Autophagie während pharmakologischer Hemmung von FLT3 untersucht. FLT3-Inhibition mittels AC220, einem FLT3-spezifischer Tyrosinkinase-Inhibitor, induzierte bzw. steigerte die autophagische Aktivität ähnlich stark wie eine direkte mTOR-Inhibition. Dies ließ sich im Zellmodell therapeutisch ausnutzen: eine Kombinationsbehandlung mit AC220 und einem lysosomalen Autophagie-Inhibitor zeigte eine synergistische antiproliferative Wirkung. Dies stellt möglicherweise einen neuen rationalen Kombinationsbehandlungsansatz für die Therapie FLT3-ITD-positiver AML-Patienten dar.
Die vorliegende Arbeit stellt eine auf datenwissenschaftlichen Methoden beruhende Analyse des aktuellen Wissens über mögliche kausale Zusammenhänge zwischen therapeutischen Morphinapplikationen mit Todesfällen dar, welche auf computergestützter Extraktion von Information aus frei verfügbaren Wissensdatenbanken und der Analyse der darin enthaltenen numerischen Information besteht. Die Relevanz der vorliegenden Analyse ergibt sich aus dem weltweit breiten Einsatz von Morphin zur Behandlung starker Schmerzen und der immer wieder vorkommenden Todesfälle während einer Morphintherapie, die regelmäßig zu Gerichtsverfahren mit den verschreibenden Ärzten als Angeklagte führen.
Morphin ist ein Opioid und zählt zu den starken Analgetika der WHO Stufe III. Bei der Applikation von Morphin kann es neben der gewünschten Analgesie auch zur Abflachung der Ventilation bis hin zum fatalen Atemstillstand kommen. In der Literatur wird die Inzidenz von Morphin-assoziierten Todesfällen mit 0,3 bis 4% angegeben. So kommt es in einigen Fällen auch zu strafrechtlichen Ermittlungen und Gerichtsverfahren mit dem Verdacht der vorsätzlichen Tötung oder sogar des Mordes. Die Frage, ob eine Morphinapplikation Ursache für den Tod eines Patienten war, ist nicht einfach zu beantworten, was mit einigen Besonderheiten von Opioid-Analgetika im Allgemeinen und von Morphin im Besonderen zusammenhängt. Für Morphin existieren z.B. keine genau definierten maximalen Dosen. Es wurden bisher lediglich Empfehlungen ausgesprochen, abhängig auch davon, ob ein Patient noch „opioid-naiv“ ist oder bereits Opioide einnimmt und damit ein Gewöhnungseffekt eingetreten ist, welcher neben der Analgesie die Gefahr des Atemstillstandes verringert. Grundsätzlich gilt immer, die Dosis so gering wie möglich und so hoch wie nötig zu halten. Die Dosis wird an die Schmerzintensität adaptiert, wobei nach keine maximal erlaubte Dosis existiert. Besonders bei terminal kranken Patienten ist die Kausalität zwischen therapeutischer Morphinapplikation und dem Tode oft fraglich, und es werden regelmäßig Gutachten eingeholt, die meistens von Rechtsmedizinern, Anästhesisten und klinischen Pharmakologen erstellt werden.
In diesen Gutachten müssen viele Faktoren, die die Wirkungen von Morphin bis hin zum letalen Ausgang beeinflussen können, diskutiert wurden, und die daher Hauptinhalt der vorliegenden Arbeit sind. Dazu gehören die verabreichten Morphindosen und die Konzentrationen von Morphin und seiner Metabolite im Blut, aber auch Charakteristika des Patienten, wie Alter, Vorerkrankungen wie z.B. Leber- oder Niereninsuffizienz, Komedikationen, oder pharmakogenetische Faktoren. Darüber hinaus spielt für die zeitliche Zuordnung einer Morphingabe mit dem Tode die verzögerte Verteilung Morphins an seinen Wirkort (das zentrale Nervensystem) eine wichtige Rolle.
Eine weitere Schwierigkeit, die sich bei Morphin-assoziierten Toden darstellt, ist die oft zur Debatte stehende verabreichte Morphindosis, die nachträglich aus den gemessenen Konzentrationen im Blut des Verstorbenen rekonstruiert werden soll, was oft nicht sicher möglich ist. Die postmortal gemessenen Konzentrationen von Morphin unterliegen relevanten Veränderungen aufgrund postmortaler Flüssigkeitsumverteilung oder des Zerfalls von Morphin, aber auch als Folge von Veränderungen während der Lagerung der Proben.
In unserer Analyse und Auswertung der vorhandenen Literatur zu diesen Themen kamen wir zu dem Ergebnis, dass es gegenwärtig praktisch sehr schwer ist, eine Morphindosis oder -konzentration sicher mit dem Tod eines Patienten in Verbindung zu bringen. Somit bleibt jeder Todesfall individuell und kontext-abhängig und erfordert die Berücksichtigung weiterer Aspekte bei der der strafrechtlichen Aufarbeitung. Zudem kamen wir zu dem Entschluss, dass angesichts dieser bereits seit langen bekannten Problemen mit Morphin und aber auch anderen Opioiden (siehe “opioid crisis” in den USA), die Entwicklung von sichereren stark wirksamen Analgetika als Ersatz für Opioide dringlich ist.
Das Prostatakarzinom ist in Europa die häufigste Krebserkrankung des Mannes. Im metastasierten, kastrationsresistenten Zustand gibt es wenige Therapieoptionen. Als neuere Therapiemöglichkeit hat sich in den letzten Jahren die Radioligandentherapie (RLT) mit 177Lu-PSMA-617 hervorgetan. Die Myelosuppression ist allerdings ein potentiell dosis-limitierender Faktor bei der Radioligandentherapie.
Ziel der vorliegenden Arbeit war es, das Auftreten, den Schweregrad und die Reversibilität von hämatotoxischen Nebenwirkungen in einer großen Patientenkohorte zu unter-suchen, die sich einer RLT mit 177Lu-PSMA-617 bei metastasiertem kastrationsresistentem Prostatakarzinom (mCRPC) unterzogen. Nach deskriptiver Analyse des behandelten Patientenkollektivs erfolgte die Identifikation von prädisponierenden Faktoren für das Eintreten von hämatologischen Nebenwirkungen. Dabei wurden insbesondere der Beitrag von Vortherapien, Behandlungsaktivität, Ausmaß der Knochentumorlast sowie eine prätherapeutisch bestehende Zytopenie berücksichtigt.
Die RLT wurde bei 140 Patienten durchgeführt, die insgesamt 497 Zyklen erhielten. Eine mittlere Aktivität von 6.9±1.3 GBq 177Lu-PSMA-617 pro Zyklus wurde in einem Median von 3 Behandlungszyklen verabreicht. Die mittlere kumulative Aktivität betrug 24.6±15.9 GBq. Hämatologische Parameter wurden bei Studienbeginn, vor jeder Behandlung, 2 bis 4 Wochen danach und während der gesamten Nachbeobachtung gemessen. Die Toxizität wurde anhand der Common Terminology Criteria for Adverse Events v5.0 eingestuft.
Dabei zeigten sich signifikante (Grad ≥3) hämatologische unerwünschte Ereignisse bei 13 Patienten (9.3%). Davon 10 mit Anämie (7.1%), 5 mit Leukopenie (3.6%) und 6 mit Thrombozytopenie (4.3%). Die Hämatotoxizität war bis zu einem medianen Follow-up von 8 Monaten bei allen bis auf zwei Patienten, die innerhalb von weniger als 3 Monaten nach der RLT an einer Krankheitsprogression starben, reversibel auf Grad ≤2. Die Myelosuppression war signifikant häufiger bei Patienten mit vorbestehender Zytopenie Grad ≥2 oder hoher Knochentumorlast (disseminiert oder diffus). Eine vorangegangene taxan-basierte Chemotherapie war ebenfalls mit einer erhöhten Inzidenz signifikanter Hämatotoxizität assoziiert, während die Behandlung mit 223Ra-Dichlorid, die kumulative RLT-Behandlungsaktivität und die Aktivität pro Zyklus nicht signifikant korreliert waren.
Die vorliegenden Ergebnisse zeigen somit, dass hämatologische Nebenwirkungen nach RLT eine akzeptable Gesamtinzidenz besitzen und häufig reversibel sind. Eine hohe Knochentumorlast, eine vorangegangene taxan-basierte Chemotherapie und eine Zyto-penie vor der Behandlung vom Grad ≥2 können als Risikofaktoren für die Entwicklung einer klinisch relevanten Myelosuppression angesehen werden, während die kumulative RLT-Aktivität und eine vorangegangene 223Ra-Dichlorid-Behandlung keinen signifikanten Beitrag zur Inzidenzrate zeigen. Die Fragestellung ist von unmittelbarer Relevanz, da die Erkenntnisse entscheidend für die individuelle Risikoeinschätzung von Patienten mit mCRPC bezüglich der 177Lu-PSMA-617 beitragen.
Natural scene responses in the primary visual cortex are modulated simultaneously by attention and by contextual signals about scene statistics stored across the connectivity of the visual processing hierarchy. Here, we hypothesized that attentional and contextual top-down signals interact in V1, in a manner that primarily benefits the representation of natural visual stimuli, rich in high-order statistical structure. Recording from two macaques engaged in a spatial attention task, we found that attention enhanced the decodability of stimulus identity from population responses evoked by natural scenes but, critically, not by synthetic stimuli in which higher-order statistical regularities were eliminated. Population analysis revealed that neuronal responses converged to a low dimensional subspace for natural but not for synthetic images. Critically, we determined that the attentional enhancement in stimulus decodability was captured by the dominant low dimensional subspace, suggesting an alignment between the attentional and natural stimulus variance. The alignment was pronounced for late evoked responses but not for early transient responses of V1 neurons, supporting the notion that top-down feedback was required. We argue that attention and perception share top-down pathways, which mediate hierarchical interactions optimized for natural vision.
Herzkreislauferkrankungen führen weltweit weiterhin die Liste der häufigsten Todesursachen an. Dem frühzeitigen Erkennen kardialer Veränderungen kommt daher eine besondere Bedeutung zu. Das Erkennen subklinischer Veränderungen des Myokards mit Hilfe des myokardialen T1 Mappings und der Strain Analyse in der kardialen MRT mit der Bestimmung der Korrelation von T1 Mapping und Veränderung des myokardialen Strains war das primäre Ziel der vorliegenden Arbeit. Das native T1 Mapping sollte dabei diffuse kardiale Fibrose erkennen, während die Strainparameter zur Analyse der subtilen kardialen Funktion dienten. Wenn eine vermehrte diffuse Fibrose eine Einschränkung der myokardialen Funktion bedeutet, so müssten sich das native T1 Mapping und die Strainparameter gleichsinnig verändern.
Eingeschlossen in die Untersuchung wurden Patienten, die im Rahmen ihres Klinikaufenthaltes an der Kerckhoff-Klinik Bad Nauheim ein kardiales MRT erhielten und ihre Zustimmung zur Aufnahme in das geführte Register gaben. Alle durchgeführten MRT-Untersuchungen besaßen dabei eine medizinische Indikation. Die Untersuchung umfasste eine Blutentnahme sowie die Bildakquisition und das Ausfüllen eines Fragebogens. In der anschließenden Bildauswertung wurden das native T1 Mapping, eine Featuretrackinganalyse, sowie die reguläre diagnostische Beurteilung durch einen Facharzt durchgeführt. Die Patienten wurden anschließend nach ihren Diagnosen in Gruppen eingeteilt. Ein Jahr nach ihrer Untersuchung wurden die Patienten kontaktiert und erneut befragt, um den Anteil an „major adverse cerebro- and cardiovasculare events“ (MACCE) und der Gesamtsterblichkeit am Gesamtkollektiv festzustellen.
Es zeigten sich signifikante Zusammenhänge zwischen nativem T1 Mapping und den Strainparametern, ebenso wie signifikante Zusammenhänge von schwächerer Effektstärke zwischen ECV und den Strainparametern. Das erhöhte native T1 Mapping ging mit verringerten Strainparametern einher, was auf eine vermehrte Fibrose schließen lässt. In der Überlebensanalyse bezüglich des allgemeinen Überlebens zeigte sich nur das native T1 Mapping als unabhängiger Prädiktor, während bei den MACCE zusätzlich auch die Anzahl der betroffenen Segmente im „late gardolinium enhancement“ (LGE) einen unabhängigen Prädiktor darstellten. Der globale longitudinale Strain (GLS) verfehlte bezüglich der MACCE knapp das Signifikanzniveau, zeigte aber eine Tendenz zur Signifikanz.
Natives T1 Mapping und, in begrenztem Maße, möglicherweise auch der Strain haben eine besondere Rolle in der Diagnostik und können bereits früh kardiale Veränderungen detektieren. Da die erhöhten T1 Zeiten als Marker für morphologische Veränderungen mit den Strains als funktionelle Parameter korrelierten, lässt sich spekulieren, dass Fibrose eine Einschränkung der Funktion bedingt.

Abstract
Atherosclerosis and numerous other cardiovascular diseases develop in an age-dependent manner. The endothelial cells that line the vessel walls play an important role in the development of atherosclerosis. Non-coding RNA like microRNAs and long non-coding RNAs are known to play an important role in endothelial function and are implicated in the disease progression. Here, we summarize several microRNAs and long non-coding RNAs that are known to have an altered expression with endothelial aging and discuss their role in endothelial cell function and senescence. These processes contribute to aging-induced atherosclerosis development and by targeting the non-coding RNAs controlling endothelial cell function and senescence, atherosclerosis can potentially be attenuated.
Background: Prostate cancer is a major health concern in aging men. Paralleling an aging society, prostate cancer prevalence increases emphasizing the need for efcient diagnostic algorithms.
Methods: Retrospectively, 106 prostate tissue samples from 48 patients (mean age,
66 ± 6.6 years) were included in the study. Patients sufered from prostate cancer (n = 38) or benign prostatic hyperplasia (n = 10) and were treated with radical prostatectomy or Holmium laser enucleation of the prostate, respectively. We constructed tissue microarrays (TMAs) comprising representative malignant (n = 38) and benign (n = 68) tissue cores. TMAs were processed to histological slides, stained, digitized and assessed for the applicability of machine learning strategies and open–source tools in diagnosis of prostate cancer. We applied the software QuPath to extract features for shape, stain intensity, and texture of TMA cores for three stainings, H&E, ERG, and PIN-4. Three machine learning algorithms, neural network (NN), support vector machines (SVM), and random forest (RF), were trained and cross-validated with 100 Monte Carlo random splits into 70% training set and 30% test set. We determined AUC values for single color channels, with and without optimization of hyperparameters by exhaustive grid search. We applied recursive feature elimination to feature sets of multiple color transforms.
Results: Mean AUC was above 0.80. PIN-4 stainings yielded higher AUC than H&E and
ERG. For PIN-4 with the color transform saturation, NN, RF, and SVM revealed AUC of 0.93 ± 0.04, 0.91 ± 0.06, and 0.92 ± 0.05, respectively. Optimization of hyperparameters improved the AUC only slightly by 0.01. For H&E, feature selection resulted in no increase of AUC but to an increase of 0.02–0.06 for ERG and PIN-4.
Conclusions: Automated pipelines may be able to discriminate with high accuracy between malignant and benign tissue. We found PIN-4 staining best suited for classifcation. Further bioinformatic analysis of larger data sets would be crucial to evaluate the reliability of automated classifcation methods for clinical practice and to evaluate potential discrimination of aggressiveness of cancer to pave the way to automatic precision medicine.

Individuell erstellte Pläne für Betreuende, Angehörige, Aufsichtspersonen und den Rettungsdienst sollen im Falle eines akuten Anfalls geeignete Handlungsanweisungen geben. Bisher gibt es hierfür im deutschsprachigen Raum keine standardisierten Vorlagen. Mit den Handlungsplänen bei epileptischen Anfällen für Laien (HEAL) bzw. Therapeuten (HEAT) werden hier 2 Formulare vorgestellt, die zum einen eine standardisierte Grundlage bieten und andererseits leicht auf den individuellen Bedarf angepasst werden können.

Caspase-2 is the evolutionary most conserved member of the caspase family and was shown to be involved in genotoxic stress induced apoptosis, control of aneuploidy, and ageing related metabolic changes. However, its role in apoptosis seems redundant due to the observation, that knockout does not inhibit apoptotic signalling exclusively. Instead, knockout of caspase-2 leads to tumor susceptibility in vivo, which led to the assumption, that caspase-2 has non-apoptotic functions and can act as a tumor suppressor. The underlying mechanism of the tumor suppressor activity of caspase-2 has not been clarified so far. Furthermore, caspase-2, has a prominent, and as pro-enzyme exclusive localisation in the nucleus and other subcellular compartments, implicating a distinct and location specific role.
In this study, a novel caspase-2 specific substrate, termed p54nrb, was identified. P54nrb is harbouring a caspase-2 specific cleavage site at the aspartate residue D422, and cleavage of p54nrb leads apparently to disruption of its putative DNA binding domain at the C-terminus.
P54nrb is a nuclear multifunctional RNA and DNA binding protein, known for roles in transcriptional regulation, DNA unwinding and repair, RNA splicing, and retention of defective RNA. Overexpression of p54nrb has been observed in several human cancers, such as cervix carcinoma, melanoma, and colon carcinoma.
Data from this study revealed, that depletion of p54nrb in tumor cell lines results in a loss of resistance to drug induced cell death and to reduced capability of anchorage independent growth, which is functionally equivalent to a reduced tumorigenic potential. Meanwhile, p54nrb depletion alone is not cytotoxic.
The investigation of p54nrb dependent gene regulations by high resolution quantitative proteomics uncovered an altering expression of multiple tumorigenic genes. For two of these candidates, the tumorigenic protease cathepsin-Z and the anti-apoptotic gelsolin, p54nrb dependent expression was detected universally in all three investigated tumor cell lines, cervix carcinoma, melanoma, and colon carcinoma. Additionally, a direct interaction of p54nrb with the cathepsin Z and gelsolin encoding DNA, but not with their corresponding mRNA, could be demonstrated.
Conjointly, this study unveils a novel mechanistic feature of caspase-2 as a tumor suppressor. The caspase-2—p54nrb axis can orchestrate the levels of several tumorigenic proteins and thereby determine the cell death susceptibility and long-term tumor survival. These findings might be of great value for future therapeutic interventions and for overcoming drug resistance of tumors.
Neuroendokrine Tumoren (NET) sind eine seltene Krankheit mit einem breitgefächerten heterogenen Erscheinungsbild, wodurch sich die Diagnose der Tumoren aus einer Vielzahl aus Gründen häufig um Jahre verzögert (1). In dieser Arbeit analysierten wir einen großen Datensatz in einem tertiären Referenzzentrum (UKF) von 1984-2019, um die Symptomatik vor der Diagnose des Tumors sowie den Zeitraum von der Tumormanifestation bis zur Diagnose weiter zu klären. Für die deskriptiven Analysen kamen SPSS, Cox-Regression und Log-Rank-Test zur Anwendung.
Insgesamt schloss die retrospektive Studie 488 gastroenteropankreastische (GEP)-NET mit 486 Patienten ≥ 18 Jahren ein, wovon knapp mehr als die Hälfte männlich (52,9%) waren. Das mittlere Alter bei Erstdiagnose (ED) betrug 58 Jahre (477/486, 9 unbekannt). Die häufigsten Primärtumorlokalisationen stellten Pankreas (143/488 Patienten) und Dünndarm (145/488 Patienten) dar. Die Mehrheit der NET waren langsam wachsende G1-Tumoren mit einem Ki67 < 3% (155/330). Die Hälfte der Patienten entwickelten im Verlauf Fernmetastasen, wobei die meisten bereits bei der ED vorlagen und insbesondere die Leber als Metastasierungsorgan dominierte. Bei mehr als 60% der Patienten konnten Angaben zur klinischen Symptomatik vor der ED detektiert werden, wovon wiederum mehr als die Hälfte symptomatisch waren. 42% der symptomatischen Patienten zeigten NET-spezifische Symptome (Bauchschmerzen 77/128; 60,2%, Durchfall 51/128; 39,8%, Flush 19/128; 14,8%, Karzinoidsyndrom 8/128; 6,3% Tachykardie 6/128; 4,7%). In der primären bildgebenden Diagnostik dominierten konventionelle Bildgebungen wie Sonographie und Computertomographie (CT), wobei nuklearmedizinische Diagnostik eine Seltenheit darstellte. Mehr als 30% der Tumoren wurden als Zufallsbefunde im Rahmen einer bildgebenden Diagnostik oder Operation diagnostiziert. Die Mehrheit der Patienten stellte sich initial außerhalb unserer Klinik vor, nur etwa 15% wurden innerhalb unserer Klinik insbesondere in der Gastroenterologie vorstellig, wo der NET diagnostiziert wurde.
Die Phase von der Tumormanifestation bis zur ED aller NET betrug im Median 17 Tage. Das Vorhandensein von Fernmetastasen sowie Symptomen führte zu keiner signifikanten Kürzung der Phase und einer schnelleren ED des NET (Median 65,5 vs. 90 Tage, p = 0,4).
Selective attention implements preferential routing of attended stimuli, likely through increasing the influence of the respective synaptic inputs on higher-area neurons. As the inputs of competing stimuli converge onto postsynaptic neurons, presynaptic circuits might offer the best target for attentional top-down influences. If those influences enabled presynaptic circuits to selectively entrain postsynaptic neurons, this might explain selective routing. Indeed, when two visual stimuli induce two gamma rhythms in V1, only the gamma induced by the attended stimulus entrains gamma in V4. Here, we modeled induced responses with a Dynamic Causal Model for Cross-Spectral Densities and found that selective entrainment can be explained by attentional modulation of intrinsic V1 connections. Specifically, local inhibition was decreased in the granular input layer and increased in the supragranular output layer of the V1 circuit that processed the attended stimulus. Thus, presynaptic attentional influences and ensuing entrainment were sufficient to mediate selective routing.
Ribosomes translate the genetic code into proteins. Recent technical advances have facilitated in situ structural analyses of ribosome functional states inside eukaryotic cells and the minimal bacterium Mycoplasma. However, such analyses of Gram-negative bacteria are lacking, despite their ribosomes being major antimicrobial drug targets. Here we compare two E. coli strains, a lab E. coli K-12 and human gut isolate E. coli ED1a, for which tetracycline exhibits bacteriostatic and bactericidal action, respectively. The in situ ribosome structures upon tetracycline treatment show a virtually identical drug binding-site in both strains, yet the distribution of ribosomal complexes clearly differs. While K-12 retains ribosomes in a translation competent state, tRNAs are lost in the vast majority of ED1a ribosomes. A differential response is also reflected in proteome-wide abundance and thermal stability assessment. Our study underlines the need to include molecular analyses and to consider gut bacteria when addressing antibiotic mode of action.
