Auf dem Weg zur multikulturellen Gesellschaft besteht Bedarf, die psychologischen Komponenten verschiedener Migrantengruppen zu erfassen. In Deutschland sind 83.598 Mitmenschen mit chinesischer Staatsbürgerschaft verzeichnet (StatBA, 2009). Werden die Eingebürgerten Übersee-Chinesen mit einbezogen, beläuft sich die Zahl an gesamtchinesischen Mitmenschen auf 135.000 (OCAC, 2009). Als einzige Anhaltspunkte über im Ausland lebende Chinesen dienen Studien aus dem angloamerikanischen Sprachraum. Menschen chinesischen Ursprungs mit westlichem Intellekt erhalten dort die Bezeichnung „Banane“, welche eine gelbe Hülle mit einem weißen Inneren illustriert. Die vorliegende Studie beschäftigt sich erstmalig mit den psychologischen Komponenten der in Deutschland lebenden chinesischen Mitbürger. Welchen Einfluss nimmt die Anpassung an das europäische Umfeld gekoppelt mit der chinesischen Internalisierung auf erinnertes elterliches Erziehungsverhalten, Persönlichkeitsstruktur, Erleben von Emotionen und Körpererleben? 154 chinesisch-stämmige Studierende aus ganz Deutschland wurden für verschiedene standardisierte psychometrische Testverfahren rekrutiert. Ein eigens konzipierter Fragebogen diente zur Messung der asiatischen und europäischen Identität. Wie vorherige Studien (Chao & Aque, 2009) aussagten, zeigten sich in der vorliegenden Stichprobe Züge des autoritativen Erziehungsstils. Im FEE (Fragebogen zum erinnertenelterlichen Erziehungsverhalten) (Schumacher, Eisemann, & Brähler, 1999) erhielten die Übersee-chinesischen Probanden neben einer höheren mütterlichen Strenge auch höhere Werte in der elterlichen Kontrolle und Überbehütung als die deutsche Normstichprobe. Je mehr mütterliche „Ablehnung und Strafe“ und elterliche „Kontrolle und Überbehütung“ die Probanden perzipierten, desto chinesischer sieht der heutige Lebensstil aus. Kongruent mit den Ergebnissen von McCrae et al. (1998) schnitten Übersee-Chinesen im NEO-FFI (Borkenau & Ostendorf, 1993) mit niedrigeren Werten in der Offenheit und höheren Werten in der Verträglichkeit ab. Anders als bei Eap et al. (2008) zeigten Chinesen in Deutschland höhere Werte in der Gewissenhaftigkeit und Extraversion. Diese hingegen korrelierten positiv mit der europäischen Identität. In der MSWS (Multidimensionale Selbstwertskala) (Schütz & Sellin, 2006) erzielten die Übersee-Chinesen einen niedrigeren allgemeinen Selbstwert als die deutsche Normgruppe. Außerdem empfanden die Übersee-Chinesen weniger eigene Selbstwertschätzung im sozialen Kontakt zu anderen, im Umgang mit Kritik und im leistungsbezogenen Bereich. Überraschenderweise erhielten die Probanden einen höheren Selbstwert in der Sportlichkeit. Die SEE (Skalen zu Erleben von Emotionen) (Behr & Becker, 2004) zeigten, dass die körperliche Symbolisierung von Gefühlen bei der chinesischen Stichprobe niedriger war als bei der deutschen Norm. Dies hing mit der Ausprägung der europäischen Identität zusammen. Negativ korrelierte die europäische Identität auch mit der Regulation von Emotionen. Im FBeK (Fragebogen zur Beurteilung des eigenen Körpers) (Strauß & Richter-Appelt,1995) ging das Körpererleben der Übersee-Chinesen mit weniger Selbstbewusstsein für die eigene Attraktivität einher. Je mehr die Lebensart zum Asiatischen tendierte, desto weniger äußerte sich die Akzentuierung des äußeren Erscheinungsbildes und desto höher erschien die Unsicherheit. Zusammenfassend weisen die Ergebnisse darauf hin, dass die Auslebung einer bikulturellen Identität Folgen für die psychologische Entwicklung eines Individuums mit sich bringen, welche beispielsweise für die Arbeit bei Patienten im psychotherapeutischen Prozess mit berücksichtigt werden sollten.
"Ciaglia Blue Rhino Punktionstracheotomie" : perioperative Komplikationen und Langzeitergebnisse

Im klinischen Alltag der Intensivstationen haben sich in den letzten Jahren verschiedene Verfahren der perkutanen Tracheotomie aufgrund ihrer einfachen Durchführbarkeit und dem geringen Komplikationsrisiko gegenüber dem chirurgischen Verfahren durchgesetzt. Die Blue Rhino Technik nach Ciaglia ist eine Methode, bei der zur Anlage des Tracheostomas nur ein einzelner Dilatationsschritt mittels eines, mit einer speziellen hydrophilen Beschichtung versehenen, gebogenen Dilatators notwendig ist. Die vorliegende Arbeit zeigt die Auswertung der Daten von 50 Patienten, die nach dieser Methode tracheotomiert wurden. Die Evaluation beinhaltet neben der Auswertung der peri- und postoperativen Komplikationen, die Ergebnisse einer telefonischen Befragung von 19 Patienten anhand eines standardisierten Fragebogens, hinsichtlich ihrer subjektiven Einschätzung bezüglich Veränderungen der Stimme, Luftnot, Schluckbeschwerden sowie dem kosmetischen Ergebnis des verschlossenen Tracheostomas. Die Ergebnisse wurden mit den in der Literatur vorhandenen Daten der verschiedenen perkutanen Verfahren verglichen, um potentielle Vorund Nachteile der Blue Rhino Technik zu eruieren. Es konnte gezeigt, dass bei der Blue Rhino Methode im Vergleich zu anderen Verfahren keine signifikant höhere perioperative Komplikationsrate auftrat. Die in verschiedenen Arbeitenbeschriebene Häufung an Trachealspangenfrakuren konnten auch wir in unserem Patientengut finden. Einen Beweis für die klinische Relevanz dieser Beobachtung wurde aber bisher in keiner Studie erbracht. Im Gegensatz dazu kann es als gesichert angesehen werden, dass ein höherer BMI ein Risikofaktor ist, der mit einer Erhöhung der perioperativen Komplikationsrate einhergeht. Auch in unserer Untersuchung traten die beiden einzigen schwerwiegenden Komplikationen bei Patienten mit einem BMI deutlich über 30 kg/m2 auf. Postoperative Komplikationen wie Blutungen oder Infektionen des Stomas konnten wir in unserem Patientenkollektiv nicht beobachten. Nach allgemeiner Studienlage treten diese Komplikationen bei den perkutanen Verfahren auch nur in Einzelfällen auf. Die bei der Befragung ein Jahr nach der Tracheotomie gefundenen Veränderungen der Stimme, Heiserkeit und das Vorliegen von Luftnot in verschiedenen Ausprägungen lassen sich bei den meisten Patienten auf ihre Vorerkrankungen zurückführen oder sind Ausdruck einer Schädigung der Stimmbänder durch die vorangegangene orotracheale Intubation. Nur bei 2 Patienten konnten wir eine Trachealstenose nicht sicher ausschließen. Einen schlüssigen Nachweis, dass eine Stenose bei Patienten nach Tracheotomie ursächlich auf die Tracheotomie alleine zurückzuführen ist, konnten wir bei Durchsicht der Literatur nicht finden. Das kosmetische Ergebnis beurteilten die meisten unserer Patienten (79%) als sehr gut bis gut. Unter Beachtung der absoluten Kontraindikationen, die für alle perkutanen Verfahren gültig sind, erfüllt die Blue Rhino Methode nach Ciaglia hinsichtlich der leichten Durchführbarkeit und der niedrigen peri- und postoperativen Komplikationsraten alle Anforderungen, die an ein perkutanes Verfahren gestellt werden.
Vasointestinal peptide metabolism plays a key physiological role in multimodular levels of vasodilatory, smooth muscle cell proliferative, parenchymal, and inflammatory lung reactions. In animal studies, vasointestinal peptide relaxes isolated pulmonary arterial segments from several mammalian species in vitro and neutralizes the pulmonary vasoconstrictor effect of endothelin. In some animal models, it reduces pulmonary vascular resistance in vivo and in monocrotaline-induced pulmonary hypertension. A 58-year-old woman presented with dyspnea and mild edema of the lower extremities. A bronchoscopy was performed without any suspicious findings suggesting a central tumor or other infiltrative disease. Endobronchial ultrasound revealed enlarged pulmonary arteries containing thrombi, a few enlarged lymph nodes, and enlarged mediastinal tissue anatomy with suspicion for mediastinal infiltration of a malignant process. We estimated that less than 10% of the peripheral vascular bed of the lung was involved in direct consolidated fibrosis as demonstrated in the left upper lobe apex. Further, direct involvement of fibrosis around the main stems of the pulmonary arteries was assumed to be low from positron emission tomography and magnetic resonance imaging scans. Assuming a positive influence of low-dose radiation, it was not expected that this could have reduced pulmonary vascular resistance by over two thirds of the initial result. However; it was noted that this patient had idiopathic pulmonary arterial hypertension mixed with "acute" (mediastinal) fibrosis which could have contributed to the unexpected success of reduction of pulmonary vascular resistance. To the best of our knowledge, this is the first report of successful treatment of idiopathic pulmonary arterial hypertension, probably as a result of low-dose radiation to the pulmonary arterial main stems. The patient continues to have no specific complaints concerning her idiopathic pulmonary arterial hypertension.
Je besser Forscher es verstehen, defekte Gene zu reparieren oder beliebige Körperzellen zu reprogrammieren, desto gefahrloser wird die Gen- und Stammzell-Therapie für Patienten, die an heute noch unheilbaren Krankheiten leiden. Gleichzeitig zeichnet sich damit die Möglichkeit ab, in ferner Zukunft vielleicht das Genom kommender Generationen zu verändern oder Menschen zu klonieren. Der Internist Prof. Hubert Serve und die Politikwissenschaftlerin Dr. Anja Karnein wagen im Gespräch mit den beiden Redakteurinnen des Wissenschaftsmagazins »Forschung Frankfurt« Dr. Anne Hardy und Ulrike Jaspers einen Ausblick jenseits aller aktuellen Debatten. Sie diskutieren aber auch über die Themen, die Patienten wie Wissenschaftler zurzeit unmittelbar berühren.
Bisherige zellfreie Labormethoden und in-vitro-Diagnostik zielen darauf ab, die Zeit von der Auslösung der Gerinnung bis zur Gerinnselbildung zu messen. Sie sind aber nicht für die Messung der Wirksamkeit von rFVIIa als Medikament geeignet, das für seine hämostatische Wirksamkeit die Funktionen von Blutzellen, besonders Plättchen, benötigt. Außerdem erfassen diese Gerinnungsteste nur einen bestimmten Abschnitt der Gerinnung und ein Zeitablauffenster, welches einen geringen Anteil (<5 %) des insgesamt gebildeten Thrombins erfasst und die Dynamik der Thrombingenerierung unberücksichtigt lässt. Die Plättchen und ihre Physiologie spielen aber die Hauptrollle in der Phospholipid-Präsentation die auf der Plättchenmembran erfolgt. Dies ist Voraussetzung für eine individuell ausreichende Thrombinbildung. Die Erfassung des endogenen Thrombinpotentials (ETP) in plättchenreichem Plasma, wie sie in dieser Arbeit verwendet wurde, überwindet diese methodischen Nachteile der etablierten Teste. Damit lässt sich eine individuelle Wirkungs- und Risikovorhersage erstellen, da hämostaserelevante Charakteristika wie z.B. Erkrankungen, Komedikation, Geschlecht, Umwelt- und lebensstilabhängige Faktoren, in die Untersuchungen einbezogen werden. In dieser Arbeit wurde mit dem Thrombingenerierungstest (TGT) eine Methode etabliert und evaluiert, die inter- und intraindivuellen Unterschiede in der Thrombinbildung in plättchenreichem Plasma (PRP) zeigt. Ausgehend von den dargestellten in-vitro Ergebnissen mit gesunden Probanden wurde die Wirksamkeit von rFVIIa im TGT, zusätzlich vergleichend und validierend mit Hilfe der Durchflusszytometrie (FACS), bei Patienten mit Thrombasthenie Glanzmann untersucht. Diese sind aufgrund einer thrombozytenfunktionsstörungsbedingten Blutungsneigung in den plasmatischen Gerinnungstesten unauffällig. Evaluiert wurden hetero- und homozygote Patienten mit unterschiedlichen zugrundeliegenden Mutationen. Zuerst wurden Proben von einer Familie in-vitro untersucht, dann eine Patientin, die im Rahmen eines zahnärztlichen Eingriffs (ex-vivo-in-vitro) unter Einbindung der klinischen Beobachtung überwacht wurde. Der TGT zeigt eine deutliche prohämostatische Wirksamkeit von rFVIIa bei allen Probanden und Patienten, FACS erfasst die zugrundeliegenden Zellfunktionen. Dabei werden Unterschiede zwischen gesunden Probanden, heterozygoten und homozygoten Patienten aufgezeigt, die diagnostisch und therapeutisch verwertbar sind. Unterschiedliche Mutationen und weitere ursächliche thrombozytäre Mechanismen schlagen sich in entsprechenden Änderungen der gemessenen FACS-Parameter nieder, die zum großen Teil mit den TGT-Werten für ETP und PEAK korrelieren. Die TGT-Werte zeigen, gerade für die Patientin EG, deren Behandlung mit rFVIIa überwacht wurde, eine stärkere Korrelation mit der Klinik als die FACSMethode. Diese wiederum erfasst die zellfunktionsbedingten Ursachen analytischer. Die Ergebnisse dieser Arbeit zeigen, dass die Messung und Auswertung der TGT-Parameter ETP und PEAK, in geringerem Maße LT und TTP, die prohämostatische Wirkung Zellfunktions-basierter Medikamente in plättchenreichem Plasma erfasst. Damit erlaubt sie über die Charakterisierung von Gerinnungsstörungen hinaus prädiktive Aussagen über die individuell angemessene Dosierung von rFVIIa (NovoSeven®).
Mitochondrien sind die Kraftwerke unserer Zellen. In ihnen findet die Zellatmung statt, die unseren Körper mit lebenswichtiger Energie versorgt. Zusätzlich teilen sich die Zellorganellen und verschmelzen wieder miteinander im Minutentakt. Was aber passiert, wenn Teile dieses dynamischen Geflechts Defekte aufweisen? Die Antwort dazu könnte ein Protein sein, das auf zwei verschiedene Weisen in die Mitochondrien-Membranen eingebaut wird. Liegt keine kurze Form des Proteins vor, ist das ein Hinweis dafür, dass die Organellen defekt sind. Die Mitochondrien verbrennen die mit der Nahrung zugeführten Kohlenhydrate und Fette unter Verbrauch von Sauerstoff zu Kohlendioxid und Wasser. Bei diesem Vorgang, der Zellatmung, wird über eine Reihe von Proteinkomplexen ein elektrochemisches Potenzial aufgebaut, das zur Produktion des Energieträgers ATP (Adenosintriphosphat) genutzt wird. ATP kann aus den Mitochondrien abtransportiert werden und steht somit als eine Art Treibstoff für alle Stoffwechselprozesse zur Verfügung. Die Arbeit der Mitochondrien ist der Hauptgrund für unseren täglichen Sauerstoffbedarf. Außerdem tragen die Nano-Kraftwerke der Zelle dazu bei, unsere Körpertemperatur auf 37 °C aufrechtzuerhalten. Aufgrund dieser zentralen Funktionen ist es nicht verwunderlich, dass eine Reihe von Krankheiten beim Menschen durch den Funktionsverlust von Mitochondrien verursacht oder beeinflusst wird. Das sind in erster Linie neurologische oder muskuläre Erkrankungen, aber auch Diabetes, Fettleibigkeit, verschiedene Formen von Krebs und Alterungsprozesse. Folglich ist es von immenser Bedeutung zu verstehen, wie Mitochondrien funktionieren, wie sie ihre Funktionalität aufrechterhalten und gegebenenfalls repariert oder entsorgt werden können. Dem können wir am Wissenschaftsstandort Frankfurt hervorragend nachgehen, da sich einige international ausgewiesene Forschungsgruppen in den Fachbereichen Medizin, Biologie, Chemie und am Max-Planck-Institut für Biophysik mit verschiedenen Aspekten der mitochondrialen Biologie befassen. In zahlreichen interdisziplinären Kooperationen wird so versucht, dieses komplexe System besser zu verstehen.
Background: Health Authorities recommend influenza vaccination of healthcare personnel (HCP) to decrease the transmission of influenza to vulnerable patients. Recent studies have almost exclusively used quantitative questionnaires in order to identify determinants of vaccination behaviour. Interviews enable HCP to express freely why they think they are (not) willing to get vaccinated against influenza.
Methods: By means of semi-structured one-on-one interviews with 123 Belgian, Dutch and German HCP, reasons for and against vaccination, experiences with influenza vaccination, intention to get vaccinated and possible barriers, as well as willingness to advice influenza vaccination to patients were investigated. Data were processed with QSR NVivo 8.0 and analysed using a combination of a deductive and a general inductive approach.
Results: Across countries, self-protection, patient protection, and protection of family members were reported as most important reasons to get vaccinated against influenza. Reasons to not get vaccinated against influenza were fear of side effects caused by the vaccine, a low risk-perception, the disbelief in the effectiveness of influenza vaccination, organizational barriers, misconceptions, and undefined negative emotions.
Conclusions: The social cognitive variables underlying the decision of HCP to get vaccinated against influenza (or not) seem to be similar in Belgium, Germany, and the Netherlands, even though some differences surfaced. A quantitative investigation of those social cognitive variables is needed in order to determine the importance of the social cognitive variables in explaining the intention to get vaccinated and the importance of the similarities and differences between countries that have been found in this study.
Psychische Störungen überschreiten alle Grenzen – es gibt sie in allen Kulturen, zu allen Zeiten, in allen soziodemografischen Schichten und in jedem Lebensalter. Sie sind häufige Themen sowohl in Talkshows, Fernsehserien und Illustrierten als auch in Literatur, Theater und bildender Kunst. Jeden können sie treffen, und beinahe jeder kennt zumindest eine Person, die an einer klinisch bedeutsamen psychischen Störung leidet. Zu deren Behandlung und Erforschung sowie zur Ausbildung von Psychologischen Psychotherapeuten wurde 1999 am Fachbereich Psychologie und Sportwissenschaften der Universität Frankfurt die Verhaltenstherapie-Ambulanz eingerichtet. Primäres Ziel der universitären Ambulanz ist dabei, die Forschung und Lehre des Fachs »Klinische Psychologie und Psychotherapie « mit der praktischen therapeutischen Arbeit an Klienten zusammenzuführen, um damit die Verbindung aus Forschung, Lehre, Ausbildung und Praxis herzustellen.
There is a debate over the association between low testosterone levels in body fluids and the occurrence of chronic periodontitis (CP). The aim of the present systematic review was to assess whether low testosterone levels in body fluids reflect CP. In order to identify studies relevant to the focus question: “Is there a relationship between low testosterone levels in body fluids and CP?” an electronic search without time or language restrictions was conducted up to June 2016 in indexed databases using different keywords: periodontitis, chronic periodontitis, periodontal diseases, testosterone, and gonadal steroid hormones. A total of eight studies were included in the present systematic review. The number of study participants ranged from 24 to 1,838 male individuals with ages ranging from 15 to 95 years. Seven studies measured testosterone levels in serum, two studies in saliva, and one study in gingiva. Four studies reported a negative association between serum testosterone levels and CP. Two studies reported a positive association between decreased testosterone levels in serum and CP. Increased levels of salivary testosterone among patients with CP were reported in one study; whereas one study reported no significant difference in the concentration of salivary testosterone between patients with and without CP. One study identified significant increase in the metabolism of testosterone in the gingiva of patients with CP. Within the limits of the evidence available, the relationship between low testosterone levels and CP remains debatable and further longitudinal studies and control trials are needed.
"Medizin bringt einem den Menschen nahe", sagt Dietmar Schranz. Schon als junger Arzt bereiste er die Welt. Er behandelte Leprakranke in Pakistan und war mit "Cap Anamur – Deutsche Not-Ärzte" in Asien. Dass er schließlich Kinderkardiologe wurde, verdankt er vier geistigen Vätern. Heute ist er selbst für viele Kardiologen weltweit zu einer prägenden Figur geworden.
Bluttransfusionen ermöglichen es jährlich tausenden von Menschen das Leben zu retten. Allerdings bringt ein unreflektierter Einsatz auch zahlreiche Nachteile mit sich. Patient Blood Management (PBM) beschäftigt sich damit, das optimale Kosten-Nutzen-Verhältnis dieser Maßnahme auszuschöpfen.
Das Programm verfolgt einen multimodalen Ansatz zur Reduktion von Transfusionen. Es zielt darauf ab präoperative Anämie zu erfassen und, wenn möglich, zu therapieren, iatrogenen Blutverlust zu reduzieren und die Anämietoleranz des Körpers maximal auszunutzen. Diese Maßnahmen wurden durch Schulungsprogramme und die Bereitstellung von Informationsmaterial begleitet, um die Sensibilität und das Wissen zu diesem Thema zu vergrößern.
Die vorliegende Arbeit befasst sich mit den Einflüssen der Einführung von PBM auf die Mortalität und Morbidität von Malignompatienten in der Viszeralchirurgie. Die retrospektive Analyse war darauf ausgerichtet, einen Unterschied von 10 % im Zweijahresüberleben vor und nach Einführung festzustellen. Hierfür wurden die Daten von 836 Patienten ausgewertet, die sich über einen Zeitraum von vier Jahren einer onkologischen Operation in der Abteilung für Allgemein- und Viszeralchirurgie der Universitätsklinik Frankfurt am Main unterzogen haben. Davon befanden sich 389 vor und 447 nach der Einführung des Projekts in Behandlung.
Das Ziel der Untersuchung bestand darin, die Sicherheit und den Nutzen von PBM in dieser speziellen Patientengruppe näher zu untersuchen. Dazu wurden der primäre Endpunkt des Zweijahresüberlebens und die sekundären Endpunkte 30-Tages- und 90-Tagesüberleben, Prozentsatz an Patienten mit Anämie, Anzahl der transfundierten Erythrozytenkonzentrate (EK) sowie das Auftreten von Komplikationen nach der Dindo-Clavien Klassifikation festgelegt.
Die erhobenen Daten zeigen ein um 13,1 % (p = < 0,001) verbessertes Überleben nach zwei Jahren in den Vergleichsgruppen vor und nach Einführung des Patient Blood Managements. Es haben 21,5 % (p = < 0,001) weniger Patienten Erythrozytenkonzentrate erhalten, zudem wurde die Gesamtsumme der transfundierten Konzentrate signifikant (p = < 0,001) reduziert.
Dabei kam es zu keinem vermehrten Auftreten von Komplikationen. Dies spricht dafür, dass die Einführung von PBM zur Verbesserung der Patientensicherheit beigetragen hat. Es hat sich gezeigt, dass Patienten ohne oder mit moderaten postoperativen Komplikationen (Dindo-Claven < IIIb) von den Veränderungen im besonderen Maße profitieren.
Eine flächendeckende Implementierung des Konzepts in den klinischen Alltag ist als sicher und empfehlenswert zu betrachten. Neben den Patienten profitiert auch das ärztliche Personal, denn im Rahmen von strukturierten Fortbildungsveranstaltungen wird tiefergehendes Wissen vermittelt und klare Handlungsempfehlungen gegeben. Dies reduziert Unsicherheiten, erhöht die Sensibilität und verringert auf diesem Weg Fehler im Transfusionsmanagement. Des Weiteren ergeben sich ökonomische Vorteile. Durch Schulungen, Anämiescreening und der Anschaffung neuer blutsparender Systeme entstehen zwar zunächst zusätzliche Kosten, allerdings stehen diese Einsparungen, durch einen geringeren EK-Verbrauch sowie geringeren Ausgaben durch eine Senkung der Morbidität, gegenüber.
Es wäre von Interesse, in zukünftigen Untersuchungen die genauen Ursachen für den beobachteten Effekt im verbesserten Langzeitüberleben zu analysieren. Mit diesem Wissen kann eine optimale Nutzung der Einflussfaktoren gewährleistet werden. Zudem sollte es auf dieser Basis möglich werden, das Patientenkollektiv, das von einer Transfusion profitiert, genauer einzugrenzen.
"PULS." - Ein Blog als Online-Magazin für Medizinstudierende der Goethe-Universität Frankfurt

Im Herbst 2009 forderten Studierende im Rahmen landesweiter Proteste auch am Fachbereich Medizin/Zahnmedizin der Goethe-Universität Frankfurt mehr Transparenz und Kommunikation zu Angelegenheiten ihres Studiums. Einen innovativen Lösungsansatz, um diesen Forderungen nachzukommen, bietet eines der Web 2.0 Werkzeuge: ein auf einer Blog-Software basierendes Online-Magazin für Studierende und andere Mitglieder des Fachbereichs.
Das öffentlich zugängliche Online-Magazin "PULS." (https://newsmagazin.puls.med.uni-frankfurt.de/wp/) wird mit einer freien Blog-Software (wordpress Version 3.1.3.) realisiert und von einer Online-Redakteurin konzipiert und geschrieben. Die Beiträge entstehen nach eigenen Recherchen sowie aus Anregungen und Gesprächen mit verschiedenen Personengruppen des Fachbereichs. Die datenschutzkonforme Auswertung der Zugriffe erfolgt über eine open-source Webanalyse-Software (Piwik). Zusätzlich werden jährlich mit dem Online-Umfrage-Tool Survey Monkey die Nutzer anonym befragt.
"PULS." ist seit dem 14.02.2010 ununterbrochen online und hat seitdem 806 Beiträge (Stand: 27.11.2012) publiziert und wird von ca. 2400 Besuchern monatlich gelesen. Das Themenspektrum ist zentriert auf die Anliegen der Frankfurter Medizin- und Zahnmedizinstudierenden. Die enge Zusammenarbeit mit verschiedenen Gruppierungen des Fachbereichs – Dekanat, Studierende und Lehrende – garantiert darüber hinaus ein fachbereichs-relevantes Themenspektrum. Das Online-Magazin begleitet komplexe Projekte und Entscheidungen mit Hintergrundinformationen und kommuniziert sie verständlich. Eine jährliche Nutzer-Evaluierung zeigt eine wachsende Leserzahl und eine sehr hohe Zustimmung für das Online-Magazin, seine Inhalte und seinen Stil. Das Web 2.0-Medium "Blog" und seine web-typische Sprache entsprechen dem Medienverhalten der Zielgruppe, d.h. den Studierenden des Fachbereichs Medizin.
"PULS." hat sich als ein geeignetes und strategisches Instrument erwiesen, um größere Transparenz, mehr Kommunikation und letztendlich eine stärkere Identifikation der Studierenden mit ihrem Fachbereich voranzutreiben.

In the context of nationwide protests 2009 also students of the faculty of medicine/dentistry at Goethe-University in Frankfurt demanded more transparency and communication. To satisfy these demands, a web 2.0-tool offered an innovative solution: A blog-based online-magazine for students and other faculty-members. The online-magazine "PULS." is realized with the share-ware blog-software (wordpress version 3.1.3) and is conceived and written by an online-journalist. "PULS." is available from https://newsmagazin.puls.med.uni-frankfurt.de/wp/. The articles are generated from own investigations and from ideas of different groups of the faculty– deanship, students and lecturers. A user-analysis is conducted with the open-source software Piwik and considers the data security. Additionally, every year an anonymous online-user-survey (Survey Monkey) is conducted. "PULS." is continuously online since 14.02.2010 and has published 806 articles (state: 27.11.2012) and has about 2400 readers monthly. The content focuses on the needs of Frankfurt medical students. The close cooperation with different groups of the faculty - deanship, students and lecturers - furthermore guarantees themes relevant to the academic faculty. "PULS." flanks complex projects and decisions with background-information and communicates them understandable. The user-evaluation shows a growing number of readers and a high acceptance for the online-magazine, its themes and its style. The web 2.0-tool "Blog" and the web-specific language comply with media habits of the main target group, the students of the faculty medicine/dentistry. Thus, "PULS." has proven as a suitable and strategic instrument. It pushes towards a higher transparency, more communication and a stronger identification of the students with their faculty.
Keine Bevölkerungsgruppe wächst so schnell wie die Gruppe der über 80-jährigen, 2050 werden es in Deutschland voraussichtlich zehn Millionen Menschen sein. Ganz ähnlich wie in vielen anderen Ländern auf allen Kontinenten, mit Ausnahme von Afrika. Aber ist die Medizin auf diese unausweichliche Entwicklung vorbereitet? »Noch nicht,« sagt Privatdozent Dr. Rupert Püllen, Altersmediziner und Chefarzt der Medizinisch-Geriatrischen Klinik der Frankfurter Diakonie- Kliniken. »Die Geriatrie führt unter den vielfältigen medizinischen Fachdisziplinen noch immer ein Schattendasein. Es mangelt an ausgebildeten Altersmedizinern ebenso wie an verlässlichen wissenschaftlichen Daten, aus denen sich evidenzbasierte Behandlungsstrategien für diese Altersgruppe ableiten lassen.« ...
Die vorliegende Arbeit beinhaltet die tierexperimentelle Evaluierung von zwei Fragestellungen innerhalb eines Tiermodels. a) Zum einen sollten die Möglichkeiten der vaskulären MR-gesteuerten Interventionen am Model der Carotis-PTA in vivo evaluiert und optimiert, und zum anderen b) die Auswirkungen von intrakraniellen CO2-Embolien bei Herzoperationen unter CO2-Protektion evaluiert werden. Insgesamt wurden an 22 Hausschweinen Gefäßinterventionen unter MRT-Kontrolle durchgeführt. Die Punktion der A. iliaca communis, als Voraussetzung für beide Fragestellungen erfolgte im Angiographieraum. Hier erfolgte auch die Positionierung der Interventionsmaterialien im Zielgefäß, der A. carotis communis. Über das Hybridsystem erfolgte der Transport in den MRT, in dem die unterschiedlichen Interventionsmaterialien getestet und die MRT-Sequenzen angepasst werden konnten. Im zweiten Teil der Arbeit wurden über einen in der A. carotis communis liegenden Katheter unterschiedliche Mengen CO2 oder Luft intracerebral appliziert. Durch eine Diffusionssequenz (DWI) konnten die hierdurch verursachten Effekte auf das Gehirngewebe beobachtet werden. Eine lokale hyperintense Zunahme in der DWI um mehr als 20% wurde als signifikant, in Bezug auf das Vorliegen einer Gehirnischämie bezeichnet. Im Rahmen der 22 Tierexperimente des interventionell, radiologischen Teils konnten die Interventionsmaterialien gut über die entwickelte Echtzeitsequenz dargestellt werden. Durch die Weiterentwicklung dieser Sequenz konnten letztendlich sowohl die Bewegungen, als auch die Öffnungsvorgänge der Ballonkatheter mittels Echtzeitbildgebung dargestellt werden. Darüber hinaus konnten Inteventionsmaterialien bezüglich ihrer Eignung für die MR-Angiographie bewertet werden. Unsere Studie konnte zeigen, dass Interventionen im Bereich der A.carotis communis unter MR-Kontrolle möglich sind. Dies gelingt bislang jedoch nur durch passive Darstellung der Intervenionsmaterialien durch deren Suszeptibilitätsartefakte. Allerdings bedarf es noch der Entwicklung von Interventionsmaterialien, hier vor allem Führungsdrähte, welche nicht magnetisch leitend sind um eine Gefährdung des Patienten auszuschließen. Im herzchirurgischen Teil konnten 16 Tiere eingeschlossen werden. Diese wurden je nach Aplikationsgas und -menge in 3 Gruppen unterteilt: Gruppe I: Gruppe II: Gruppe III: Bei allen Tieren wurden Signalanhebungen in der Diffusionssequenz in der ipsi- und kontralateralen Hemisphäre, deren Persistenz über 25 Minuten, sowie klinische Symptome registriert. Herauszustellen sind die deutlich unterschiedlichen klinischen Reaktionen der Gruppe I zu den Gruppen II und III in unserer Studie. Die Kreislaufreaktionen, die in der Gruppe I bei 4 von 5 Tieren bis zum Kreislaufstillstand reichten, wurden von deutlichen, bilateralen Signalveränderungen in der DWI begleitet. In diesen Fällen muss von einem irreversiblen, großen cerebralen Insult ausgegangen werden. Lediglich bei 2 von 11 Tieren der beiden CO2 Gruppen, waren die Signalanhebungen progredient und dehnten sich in den späten Messungen auch auf die kontralaterale Hemisphäre aus. Bei allen übrigen Tieren dieser Gruppen waren nur leichte Signalanhebungen nach 10 Minuten zu erkennen. Daraus ist zu schließen, dass mit keinen bleibenden Schädigungen der Gehirnzellen zu rechnen ist, da die Ischämiezeit deutlich unter 15 Minuten lag.
Mit den Krankheiten häuft sich im Alter auch die Zahl der einzunehmenden Medikamente. Das bringt viele Probleme mit sich. Das Institut für Allgemeinmedizin der Goethe-Universität untersucht in enger Kooperation mit der Universität Maastricht die Folgen der Multimedikation und entwickelt gemeinsam mit Hausärzten Strategien, um unerwünschte Wirkungen zu vermeiden.
Hintergrund. Die Achtung der individuellen Autonomie ist eines von vier medizinethischen Prinzipien, das im Kontext von Medizin und Forschung insbesondere in Bezug auf die informierte Einwilligung einer Person thematisiert wird. Menschen mit Demenz können aufgrund innerer oder äußerer Faktoren in ihrer Einwilligungsfähigkeit beeinträchtigt sein, was zu einer Einschränkung ihres Rechts auf Selbstbestimmung führen kann. Im diesbezüglichen Spannungsfeld zwischen Fürsorge und Autonomie soll Entscheidungsassistenz zur Ermöglichung selbstbestimmter Entscheidungen beitragen.
Zielrichtung der Arbeit. Ziel der vorliegenden Dissertation ist die Definition, Implementierung und Evaluation von Entscheidungsassistenzmaßnahmen für Menschen mit Demenz, um deren Autonomie in Entscheidungsprozessen zu unterstützen. Drei Teilprojekte umfassen die Ermittlung des internationalen Forschungsstands zu Entscheidungsassistenz bei Demenz, die Definition und Pilotierung von Unterstützungstools in der Praxis und die Analyse des individuellen Erlebens der vereinfachten Aufklärungsgespräche durch Menschen mit Demenz.
Methode. Im ersten Teilprojekt wurde eine am PRISMA-Standard orientierte systematische Literaturrecherche in Medline und PsycINFO durchgeführt. Die extrahierten relevanten Informationen wurden inhaltlich systematisiert. Aufbauend auf diesen Ergebnissen wurden im zweiten Teilprojekt konkrete Unterstützungstools definiert und in reale Aufklärungsgespräche (Lumbalpunktion) implementiert. Die Tools wurden in der Pilotierung in der Praxis sowie in einem iterativen Diskussionsprozess mit Experten weiterentwickelt. Im dritten Teilprojekt wurde das individuelle Erleben der Teilnehmer der vereinfachten Aufklärungsgespräche mittels problemzentrierter Interviews untersucht und die Daten einer qualitativen Inhaltsanalyse unterzogen.
Ergebnisse. Die Datenbankrecherche ergab initial 2348 Treffer. Nach Screenings der Titel, Abstracts und Volltexte konnten 11 Artikel eingeschlossen werden. Vier der eingeschlossenen Studien sind Interventionsstudien, die übrigen sieben qualitative Interviewstudien. Die identifizierten Unterstützungsmaßnahmen wurden zunächst den beiden Kategorien Interventionen und Strategien und anschließend unter Zuhilfenahme des Konzepts des Contextual Consents fünf komplexitätssteigernden Dimensionen einer Entscheidungssituation zugeordnet (individuelle, soziale, medizinische, informationelle und Folgendimension). Darauf aufbauend wurden im zweiten Teilprojekt acht Entscheidungsassistenzmaßnahmen abgeleitet: (1) Gesprächsstruktur, (2) Elaborierte klare Sprache, (3) Ambiente / Raumgestaltung, (4) Stichwortlisten, (5) Prioritätenkarten, (6) Visualisierung, (7) Vereinfachte schriftliche Einverständniserklärung sowie (8) Personenzentrierte Haltung des Entscheidungsassistenten (1-7: Tools, 8: Grundeinstellung). Die Tools zielen überwiegend auf eine Komplexitätsreduktion in der informationellen Dimension unter Berücksichtigung der fähigkeitsbezogenen und der bedürfnisbezogenen individuellen Dimension ab. Durch Anpassungen der Informationsdarbietung oder der kommunikativen Interaktion im Gespräch dienen sie mehrheitlich der Förderung des (Informations-) Verständnisses. Die Analyse der qualitativen Daten im dritten Teilprojekt zeigt, dass die Erfahrung der vereinfachten Aufklärungsgespräche durch drei übergreifende Themen gekennzeichnet ist. Die Kategorie Formalität versus Informationsgewinn illustriert die individuelle Bedeutung des Aufklärungsgesprächs für die Teilnehmer und deren Bewertung des Prozesses der informierten Einwilligung. Die Kategorie Wahrnehmung der Unterstützung skizziert die Bewertungen der angewandten Unterstützungstools durch die Teilnehmer. Die Kategorie Der Wahrheit ins Auge sehen müssen stellt dar, dass die erlebte Situation des vereinfachten Aufklärungsgesprächs wesentlich durch die Verdachtsdiagnose Demenz bestimmt ist, die im Rahmen aller Aufklärungsgespräche besprochen wurde.
Fazit. Bislang gibt es wenig empirische Forschung zu Entscheidungsassistenz für Menschen mit Demenz und Unterstützungsmaßnahmen werden überwiegend unsystematisch entwickelt und angewendet. Die Wirksamkeit einzelner Unterstützungsmaßnahmen kann aufgrund fehlender Interventionsstudien selten beurteilt werden. Unterstützungsmaßnahmen zielen überwiegend auf eine Komplexitätsreduktion in der Informationsdarbietung und im kommunikativen Interaktionsprozess ab, wobei sie kognitive Beeinträchtigungen und Interaktions-/ Entscheidungsbedürfnisse von Menschen mit Demenz berücksichtigen. Die definierten Tools können als erste konkret handhabbare Werkzeuge verstanden werden, die das strukturierte Leisten von Entscheidungsassistenz für Menschen mit Demenz erleichtern sollen. Sie sind übertragbar auf verschiedene Entscheidungssituationen. Eine Bewertung der Wirksamkeit der definierten Tools sollte in weiteren Entscheidungssituationen und mit größeren Stichproben weiteruntersucht werden. Die Ergebnisse der Evaluation liefern jedoch erste Hinweise darauf, dass einige Teilnehmer sich von einzelnen Tools unterstützt gefühlt haben und die anvisierte Komplexitätsreduktion in der informationellen Dimension in einigen Fällen erfolgreich war. Eine wesentliche Komplexitätssteigerung in der untersuchten Entscheidungssituation entstand durch die negative Emotionen auslösende Vermittlung einer potentiellen Demenzdiagnose (Folgendimension). Dieses Ergebnis impliziert, dass die definierte „verständnisfördernde Toolbox“ um Unterstützungsmaßnahmen zur emotionalen Entlastung von Menschen mit Demenz erweitert werden muss, da davon ausgegangen werden kann, dass vielfältige Entscheidungssituationen für Menschen mit Demenz emotional hoch belastend sind.
Die HAART hat einen Durchbruch in der Therapie der HIV-Infektion bewirkt und so zu einer drastischen Senkung der Mortalität und Morbidität geführt. Um diesen Ansprüchen weiterhin gerecht zu werden und sie bestenfalls zu übertreffen, erfordert eine ständige Weiterentwicklung der HAART mit neuen und ausgefeilteren Alternativen. Ein weiterer Schritt in diese Richtung ist die Entwicklung einer neuen Formulierung des Kombinationspräparates LPV/r (Kaletra®) von der „lipophilen Kapselform“ zur „hydrophilen Tablettenform“, aus der Wirkstoffgruppe der Proteasehemmer. Lopinavir (LPV) ist ein HIV-Proteasehemmer der mit Ritonavir (r oder RTV) als fixe Kombination (LPV/r) hergestellt wird. Der Proteasehemmer Ritonavir wird dabei in subtherapeutischer Dosierung als Booster verwendet, dadurch wird eine Verbesserung der pharmakokinetischen Eigenschaften erzielt. Der Vorteil hierbei sind die höheren Lopinavir-Plasmaspiegel die erreicht werden. Diese Kombination wird als Kaletra® (LPV/r) vermarktet.
LPV/r ist erhältlich als lipophile Kapselform (133,3/33,3mg) oder in Flüssigform (80/20mg pro ml). Beide erfordern eine kühle Lagerung und müssen mit einer fettreichen Mahlzeit eingenommen werden, um optimale Lopinavir Plasmaspiegel zu erzielen.
Durch das „Melt Extrusion (Meltrex)“ Produktionsverfahren gelang die Herstellung einer „hydrophilen Tabletteform“ (200/50mg und 100/25mg) mit verbesserter Bioverfügbarkeit. Dadurch reduzierte sich die einzunehmende Anzahl von 6 Kapseln pro Tag auf 4 Tabletten pro Tag. Zudem bedarf die LPV/r Tablette keiner Kühlung und kann nahrungsunabhängig eingenommen werden.
Ziel dieser Untersuchung war es zu prüfen, welche LPV/r (Kaletra®) Darreichungsform, Kapsel oder Tablette, in einer HAART von HIV-Patienten bevorzugt wird. Es sollte ermittelt werden, ob bei gleichbleibender Wirksamkeit kombiniert mit einer verbesserten Verträglichkeit und Handhabung (weniger Tabletten, nahrungsunabhängige Einnahme und keine Kühlung), die überwiegende Mehrzahl der HIV-Patienten sich zugunsten der LPV/r Tablette, im Sinne einer verbesserten Lebensqualität bzw. Gemütszustandes, entscheiden werden.
Dies geschah anhand einer prospektiven, nicht randomisierten Studie mit 238 HIV-infizierten Patienten, die über mindesten 16 Wochen oder länger eine LPV/r Kapsel haltige antiretrovirale Kombinationstherapie einnahmen und am Tag 0 auf LPV/r Tabletten umgestellt wurden, ohne weitere Änderungen in ihrer bisherigen HAART vorzunehmen. Der darauffolgende Beobachtungszeitraum betrug 32 Wochen. Es wurden Vorher-, Nachher-Fragebogen ausgefüllt und die Patienten unterzogen sich einer Vorher-, Nachher-Laboruntersuchung (CD4 und HI-Viruslast). Zudem wurde nach der subjektiven Präferenz gegenüber beiden Darreichungsformen (Kapsel oder Tablette) gefragt.
Zusammenfassend lässt sich sagen, dass unter der LPV/r Tablette bei gleichbleibender antiretroviraler Wirksamkeit, signifikant weniger intestinale Nebenwirkungen auftraten und daran geknüpft signifikant weniger Medikamente gegen intestinale Beschwerden eingenommen wurden. Was bei den Patienten zu einer deutlichen Präferenz der LPV/r-Tablette (71,2 %) gegenüber der LPV/r Kapsel (3,0 %) führte. Die Ergebnisse zu Lebensqualität zeigten zwar eine tendenzielle Besserung aber zusammen mit den Gemütszuständen ergaben sich hier keine signifikanten Unterschiede.
Nach der vorliegenden Untersuchung muss die LPV/r Tablette im Vergleich zur LPV/r-Kapsel, als die überlegene antiretrovirale Therapieoption in Betracht gezogen werden. Angesichts zahlreicher Einschränkungen durch die Infektion und die Notwendigkeit einer lebenslangen Therapie, kann dies, ein bedeutender Beitrag zur Therapietreue sein und dadurch den Erfolg einer HIV-Therapie wesentlich mitbestimmen.
Background: To investigate patients’ perspectives on polypharmacy and the use of a digital decision support system to assist general practitioners (GPs) in performing medication reviews. Methods: Qualitative interviews with patients or informal caregivers recruited from participants in a cluster-randomized controlled clinical trial (cRCT). The interviews were transcribed verbatim and analyzed using thematic analysis. Results: We conducted 13 interviews and identified the following seven themes: the patients successfully integrated medication use in their everyday lives, used medication plans, had both good and bad personal experiences with their drugs, regarded their healthcare providers as the main source of medication-related information, discussed medication changes with their GPs, had trusting relationships with them, and viewed the use of digital decision support tools for medication reviews positively. No unwanted adverse effects were reported. Conclusions: Despite drug-related problems, patients appeared to cope well with their medications. They also trusted their GPs, despite acknowledging polypharmacy to be a complex field for them. The use of a digital support system was appreciated and linked to the hope that reasons for selecting specific medication regimens would become more comprehensible. Further research with a more diverse sampling might add more patient perspectives.
'Skelettfund' im Keller

Bei der Identifizierung einer unbekannten, stark verwesten Leiche oder eines Skelettes ohne Hinweise auf die Identität durch die Auffindesituation spielt die Erstellung des sog. biologischen Profils eine entscheidende Rolle. Vorgestellt wird ein Leichenfund in einem mehr oder weniger frei zugänglichen Kellerabteil eines Mehrfamilienhauses. Der Leichnam war weitgehend skelettiert, das Skelett jedoch durch mumifizierte Weichteilreste noch nahezu vollständig zusammengehalten. Bei den Hinweisen auf die Identität ergaben sich insbesondere in der Altersschätzung scheinbare Widersprüche, die jedoch zufällig eine relativ genau zutreffende Schätzung lieferten. Die Überreste konnten mittels forensischer DNA-Analyse einer seit 4 Jahren vermissten 49-Jährigen zugeordnet werden. Als Todesursache wurde ein Kältetod diskutiert.
Der Fall wies eine außergewöhnliche Auffindesituation auf, die an Bilder von „Skelettfunden“ in Kriminalverfilmungen erinnerte. Derartige Befunde dürften jedoch in der Realität wohl nur sehr selten vorkommen. Darüber hinaus werden die Wichtigkeit und die Probleme der forensisch-osteologischen Untersuchungen bei der Identifizierung eines stark verwesten, unbekannten Leichnams demonstriert.
Background: The rate of caesarean sections increased in the last decades to about 30% of births in western populations. Many caesarean sections are electively planned without an urgent medical reason for mother or child. Especially in women with a foregoing caesarean section, the procedure is planned early. An early caesarean section though may harm the newborn. Our aim is to evaluate the gestational time point after the 37th gestational week (after prematurity = term) of performing an elective caesarean section with the lowest morbidity for mother and child.
Methods: This is an update of a systematic review previously carried out on behalf of the German Federal Ministry of Health. We will perform a systematic literature search in MEDLINE, EMBASE, CENTRAL and CINAHL. Our primary outcome is the rate of admissions to the neonatal intensive care unit in early versus late term neonates. We will include (quasi) randomized controlled trials and cohort studies. The studies should include pregnant women who have an elective caesarean section at term. We will screen titles and abstracts and the identified full texts of studies for eligibility. Risk of bias will be assessed with the Cochrane Risk of Bias Tool for Randomized Trials or with the Risk of Bias Tool for Non-Randomized Studies of Interventions (ROBINS-I). These tasks will be performed independently by two reviewers. Data will be extracted in beforehand piloted extraction tables. A dose-response meta-analysis will be performed.
Discussion: Our aim is to reach a higher validity in the assessment of the time point of elective caesarean sections by performing a meta-analysis to support recommendations for clinical practice. We assume to identify less randomized controlled trials but a large number of cohort studies analyzing the given question. We will discuss similarities and differences in included studies as well as methodological strengths and weaknesses.

Die in vivo MR-Protonenspektroskopie (1H-MRS) ist ein nichtinvasives Verfahren, das die Untersuchung biochemischer Substanzen beim Lebenden ermöglicht. In einer prospektiven Studie wurden Patienten, die an einer Demenz vom Alzheimer-Typ (DAT) litten, und altersentsprechende Probanden mit der 1H-MRS untersucht. Im Hirngewebe wurden Konzentrationen von Molekülen bestimmt, die Aussagen über die zelluläre Zusammensetzung ermöglichen. Das Hauptinteresse galt der Untersuchung von Glutamat und Glutamin, da ein Überangebot des Neurotransmitters Glutamat im synaptischen Spalt möglicherweise für die Neurodegeneration beim M. Alzheimer mitverantwortlich ist. Bei 29 Patienten mit einer DAT sowie 19 Probanden wurden zwei Volumina in Kortex und Marklager des Parietallappens bei 1,5 Tesla Magnetfeldstärke mit einer PRESS-Sequenz (TE = 22 ms, TR = 3 s) untersucht. Die Patienten waren kognitiv leicht bis mittelschwer beeinträchtigt (Mini Mental State Examination, MMSE, 11-27 Punkte). Die quantitative Auswertung der Spektren erfolgte nach der "phantom replacement method" mit dem Softwareprogramm LCModel. Als Kriterium für die Zuverlässigkeit der Konzentrationsangaben diente die vom Programm als %SD-Wert angegebene Auswertegenauigkeit der Einzelmessungen. Neben dem Glutamat- und Glutamingehalt wurden der Liquorgehalt im kortikalen Messvolumen und die Konzentrationen der Moleküle N-Acetyl-Aspartat und N-Acetyl-Aspartyl- Glutamat (tNAA), myo-Inositol (mI), Kreatin und Kreatinphosphat (tCr) sowie der cholinhaltigen Substanzen (tCh) bestimmt. Bei den Patienten konnten 25 aussagekräftige Spektren aus der Rindenregion und 19 aus dem Marklager des Parietallappens gemessen werden, bei den Probanden 17 Kortex- und 13 Marklager-Spektren. Glutamat und Glutamin können mit der in vivo-Spektroskopie wegen ihrer sich überlappenden Resonanzen nicht separat bestimmt werden und wurden deshalb als „Glx“ zusammengefasst. Der Glx-Gehalt betrug in der Rindenregion bei den Patienten im Durchschnitt 15,48 ± 5,15 mmol/l, bei den Probanden 13,98 ± 3,14 mmol/l. Die um 11% höhere Konzentration bei den Patienten war nicht signifikant. Die Glx-Konzentrationen im Marklager betrugen 8,16 ± 3,79 mmol/l bei den Patienten und 8,14 ± 3,71 mmol/l bei den Probanden. Nach dem Kriterium der %SD war die Zuverlässigkeit der Konzentrationsbestimmung von Glutamat und Glutamin eingeschränkt, was auf die protonenspektroskopischen Eigenschaften von Glutamat und Glutamin zurückzuführen ist. Sie erzeugen wegen ihrer gekoppelten Resonanzen breite und mehrgipflige Protonensignale, die wegen der resultierenden niedrigen Signalamplitude für LCModel von Makromolekülresonanzen oder von Artefakten, die durch ungenügende Wasserunterdrückung entstehen, schwierig abzugrenzen sind. Die %SD-Werte der Einzelmessungen von tNAA, mI, tCr und tCh waren gering, die Messergebnisse können daher als zuverlässig angesehen werden. Die tNAA-Konzentrationen in beiden Gruppen unterschieden sich weder in der grauen noch in der weißen Substanz signifikant voneinander. Im Hirnrindenvolumen zeigte sich bei den Patienten jedoch eine positive Korrelation von tNAA und MMSE-Ergebnis, also eine Reduktion von tNAA mit zunehmenden kognitiven Einbußen. Dies spricht für eine Abnahme des neuronalen Volumenanteils mit fortschreitender Erkrankung. Der durchschnittliche myo-Inositolgehalt im Marklagervolumen lag in der Patientengruppe um 20% höher als bei den Probanden. Der Unterschied erreichte jedoch kein Signifikanzniveau (p = 0,09). Im Rindenvolumen war die mittlere myo-Inositolkonzentration bei den Patienten um 7% höher als in der Kontrollgruppe, auch hier war der Unterschied nicht signifikant. Der nichtsignifikante myo-Inositolanstieg lässt sich als mäßige entzündliche oder reaktive Gliaproliferation interpretieren. Die tCr-Konzentrationen in beiden Gruppen unterschieden sich nicht, jedoch bestand bei den DAT-Patienten eine positive Korrelation des tCr-Gehalts mit der MMSE-Punktzahl in der grauen Substanz des Parietallappens, die Konzentration nahm also mit zunehmendem Schweregrad der Demenz ab. Bei der durchschnittlichen tCh-Konzentration bestanden keine Gruppenunterschiede in den untersuchten Regionen. Die Patienten wiesen eine signifikante Zunahme des mittleren Liquoranteils um 16% im kortikalen Volumen auf. Dies ist als kortikale Hirngewebeatrophie in frühen und mittleren Stadien der DAT zu werten. Während die in vivo-Konzentrationen von tNAA, mI, tCr und tCh zuverlässig bestimmt werden konnten, war die Messung von Glutamat und Glutamin bei 1,5 T mit technischen Schwierigkeiten verbunden. Wegen der hohen Standardabweichungen konnte aus dieser Untersuchung keine gesicherte Aussage zu einer möglichen Konzentrationsänderung dieser Aminosäuren bei der DAT abgeleitet werden. Es ist zu erwarten, dass hier durch MR-Geräte mit 3 T Feldstärke sowie durch den Einsatz der parallelen Bildgebung ein erheblich höheres Signal/Rausch-Verhältnis und damit genauere Ergebnisse erzielt werden können.
Ziel der Nachuntersuchung war es, 10 Jahres Ergebnisse nach arthroskopischer vorderer Kreuzbandplastik mittels Patellarsehne zu erhalten. Dafür wurden 74 Patienten, 55 Männer und 19 Frauen mit einem durchschnittlichen Alter von 38 Jahren im Mittel 113 Monaten nach arthroskopischer vorderer Kreuzbandplastik mit Interferenzschraubenfixation nachuntersucht. Evaluiert wurden die IKDC Evaluation, die Tegner - Aktivitätsskala und der Lysholm - Score. Die Bandstabilität wurde instrumentell mit dem Rolimeter getestet. Die statistische Auswertung erfolgte mittels c² Test. Die Tegner - Aktivitätsskala ergab ein durchschnittliches Aktivitätsniveau von 5,2. Vor der Verletzung lag der Wert bei 6,4. Der Lysholm - Score erreichte 93,6 von 100 Punkten. Hinsichtlich der Funktion wurden 74% und hinsichtlich der Stabilität 72% der Patienten in der IKDC Evaluation als normal bzw. als fast normal bewertet. Der mit dem Rolimeter bestimmte Lachman Test war in 88% normal und in 12% fast normal. Der pivot-shift Test war in 95% der Fälle negativ. 30% der Patienten zeigten in den Röntgenaufnahmen zum Nachuntersuchungszeitpunkt deutlich degenerative Veränderungen. Es ergibt sich eine signifikante Korrelation zwischen der Zeitdauer vom Unfall bis zur operativen Versorgung und dem Arthrosegrad. Die Beschwerden an der Transplantatentnahmestelle korrelieren signifikant mit den allgemeinen Symptomen. Das Ausmaß der Arthrose korreliert signifikant mit der Mensikusschädigung. Zur Vermeidung der Folgearthrose sollte daher bei gegebener Indikation eine frühzeitige Kreuzbandversorgung und im Falle der Begleitmeniskusverletzung dessen Erhalt mittels Refixation angestrebt werden.

The 20 April 2020 marks the centenary of Dieter Janz’s birth. This issue of Zeitschrift für Epileptologie is published in his honor with the aim of tracing the work of Dieter Janz over the last five decades and summarizing new findings on the Janz syndrome (Juvenile Myoclonic Epilepsy), which is named after him.
Introduction: Epoxyeicosatrienoic acids (EETs) are able to enhance angiogenesis and regulate inflammation that is especially important in wound healing under ischemic conditions. Thus, we evaluated the effect of local EET application on ischemic wounds in mice.
Methods: Ischemia was induced by cautherization of two of the three supplying vessels to the mouse ear. Wounding was performed on the ear three days later. Wounds were treated either with 11,12 or 14,15 EET and compared to untreated control and normal wounds. Epithelialization was measured every second day. VEGF, TNF-α, TGF-β, matrix metalloproteinases (MMP), tissue inhibitors of metalloproteinases (TIMP), Ki67, and SDF-1α were evaluated immunohistochemically in wounds on day 3, 6, and 9.
Results: Ischemia delayed wound closure (12.8 days ± 1.9 standard deviation (SD) for ischemia and 8.0 days ± 0.94 SD for control). 11,12 and14,15 EET application ameliorated deteriorated wound healing on ischemic ears (7.6 ± 1.3 SD for 11,12 EET and 9.2 ± 1.4 SD for 14,15 EET). Ischemia did not change VEGF, TNF-α, TGF-β, SDF-1α, TIMP, MMP7 or MMP9 level significantly compared to control. Local application of 11,12 as well as 14,15 EET induced a significant elevation of VEGF, TGF-β, and SDF-1α expression as well as proliferation during the whole phase of wound healing compared to control and ischemia alone.
Conclusion: In summary, EET improve impaired wound healing caused by ischemia as they enhance neovascularization and alter inflammatory response in wounds. Thus elevating lipid mediator level as 11,12 and 14,15 EET in wounds might be a successful strategy for amelioration of deranged wound healing under ischemia.
Epoxyeicosatrienoic acids (EET) facilitate regeneration in different tissues, and their benefit in dermal wound healing has been proven under normal conditions. In this study, we investigated the effect of 11,12 EET on dermal wound healing in diabetes. We induced diabetes by i.p. injection of streptozotocin 2 weeks prior to wound creation on the dorsal side of the mouse ear. 11,12 EET was applied every second day on the wound, whereas the control groups received only solvent. Epithelialization was monitored every second day intravitally up to wound closure. Wounds were stained for VEGF, CD31, TGF-β, TNF-α, SDF-1α, NF-κB, and Ki-67, and fibroblasts were counted after hematoxylin-eosin stain on days 3, 6, 9, and 16 after wounding. After induction of diabetes, wounds closed on day 13.00 ± 2.20 standard deviation (SD). Local 11,12 ETT application improved wound closure significantly to day 8.40 ± 1.39 SD. EET treatment enhanced VEGF and CD31 expression in wounds on day 3. It also seemed to raise TNF-α level on all days investigated as well as TGF-β level on days 3 and 6. A decrease in NF-κB could be observed on days 9 and 16 after EET application. The latter findings were not significant. SDF-1α expression was not influenced by EET application, and Ki-67 was significantly less in the EET group on day 9 after EET application. The number of fibroblasts was significantly increased on day 9 after the 11,12 EET application. 11,12 EET improve deteriorated wound healing in diabetes by enhancing neoangiogenesis, especially in the early phase of wound healing. Furthermore, they contribute to the dissolution of the initial inflammatory reaction, allowing the crucial transition from the inflammatory to proliferative phase in wound healing.
In the systemic circulation, 11,12-epoxyeicosatrienoic acid (11,12-EET) elicits nitric oxide (NO)- and prostacyclin-independent vascular relaxation, partially through the activation of large conductance Ca2+-activated potassium (BK) channels. However, in the lung 11,12-EET contributes to hypoxia-induced pulmonary vasoconstriction. Since pulmonary artery smooth muscle cells also express BK channels, we assessed the consequences of BKβ1 subunit deletion on pulmonary responsiveness to 11,12-EET as well as to acute hypoxia. In buffer-perfused mouse lungs, hypoxia increased pulmonary artery pressure and this was significantly enhanced in the presence of NO synthase (NOS) and cyclooxygenase (COX) inhibitors. Under these conditions the elevation of tissue EET levels using an inhibitor of the soluble epoxide hydrolase (sEH-I), further increased the hypoxic contraction. Direct administration of 11,12-EET also increased pulmonary artery pressure, and both the sEH-I and 11,12-EET effects were prevented by iberiotoxin and absent in BKβ1−/− mice. In pulmonary artery smooth muscle cells treated with NOS and COX inhibitors and loaded with the potentiometric dye, di-8-ANEPPS, 11,12-EET induced depolarization while the BK channel opener NS1619 elicited hyperpolarization indicating there was no effect of the EET on classical plasma membrane BK channels. In pulmonary artery smooth muscle cells a subpopulation of BK channels is localized in mitochondria. In these cells, 11,12-EET elicited an iberiotoxin-sensitive loss of mitochondrial membrane potential (JC-1 fluorescence) leading to plasma membrane depolarization, an effect not observed in BKβ1−/− cells. Mechanistically, stimulation with 11,12-EET time-dependently induced the association of the BK α and β1 subunits. Our data indicate that in the absence of NO and prostacyclin 11,12-EET contributes to pulmonary vasoconstriction by stimulating the association of the α and β1 subunits of mitochondrial BK channels. The 11,12-EET-induced activation of BK channels results in loss of the mitochondrial membrane potential and depolarization of the pulmonary artery smooth muscle cells.
Cytochrome P450-derived epoxyeicosatrienoic acids (EETs) stimulate endothelial cell proliferation and angiogenesis. In this study, we investigated the involvement of the forkhead box, class O (FOXO) family of transcription factors and their downstream target p27Kip1 in EET-induced endothelial cell proliferation. Incubation of human umbilical vein endothelial cells with 11,12-EET induced a time- and dose-dependent decrease in p27Kip1 protein expression, whereas p21Cip1 was not significantly affected. This effect on p27Kip1 protein was associated with decreased mRNA levels as well as p27Kip1 promoter activity. 11,12-EET also stimulated the time-dependent phosphorylation of Akt and of the forkhead factors FOXO1 and FOXO3a, effects prevented by the phosphatidylinositol 3-kinase inhibitor LY 294002. Transfection of endothelial cells with either a dominant-negative or an “Akt-resistant”/constitutively active FOXO3a mutant reversed the 11,12-EET-induced down-regulation of p27Kip1, whereas transfection of a constitutive active Akt decreased p27Kip1 expression independently of the presence or absence of 11,12-EET. To determine whether these effects are involved in EET-induced proliferation, endothelial cells were transfected with the 11,12-EET-generating epoxygenase CYP2C9. Transfection of CYP2C9 elicited endothelial cell proliferation and this effect was inhibited in cells co-transfected with CYP2C9 and either a dominant-negative Akt or constitutively active FOXO3a. Reducing FOXO expression using RNA interference, on the other hand, attenuated p27Kip1 expression and stimulated endothelial cell proliferation. These results indicate that EET-induced endothelial cell proliferation is associated with the phosphatidylinositol 3-kinase/Akt-dependent phosphorylation and inactivation of FOXO factors and the subsequent decrease in expression of the cyclin-dependent kinase inhibitor p27Kip1.
Objectives: The authors sought to evaluate the performance of the Ranger paclitaxel-coated balloon versus uncoated balloon angioplasty for femoropopliteal lesions at 12 months.
Background: Drug-coated balloons (DCBs) are a promising endovascular treatment option for peripheral artery disease of the femoropopliteal segment, and each unique device requires dedicated clinical study.
Methods: The prospective, randomized RANGER SFA (Comparison of the Ranger™ Paclitaxel-Coated PTA Balloon Catheter and Uncoated PTA Balloons in Femoropopliteal Arteries) study (NCT02013193) enrolled 105 patients with symptomatic lower limb ischemia (Rutherford category 2 to 4) and stenotic lesions in the nonstented femoropopliteal segment at 10 European centers. Seventy-one patients (mean age 68 ± 8 years, n = 53 men) were enrolled in the Ranger DCB arm, and 34 patients (mean age 67 ± 9 years, n = 23 men) were assigned to the control group. Twelve-month analysis included patency, safety, and clinical outcomes and quality-of-life assessments.
Results: The DCB group had a greater primary patency rate at 12 months (Kaplan-Meier estimate 86.4% vs. 56.5%), with a significantly longer time to patency failure (log-rank p < 0.001). The estimated freedom from target lesion revascularization rate was 91.2% in the DCB group and 69.9% in the control group at 12 months, with a significantly longer time to reintervention (p = 0.010). No target limb amputations or device-related deaths occurred in either group.
Conclusions: Twelve-month results show that patency was maintained longer after Ranger DCB treatment than after conventional balloon angioplasty, and this result was associated with a low revascularization rate and good clinical outcomes.

Prostaglandin (PG) E2 (PGE2) plays a predominant role in promoting colorectal carcinogenesis. The biosynthesis of PGE2 is accomplished by conversion of the cyclooxygenase (COX) product PGH2 by several terminal prostaglandin E synthases (PGES). Among the known PGES isoforms, microsomal PGES type 1 (mPGES-1) and type 2 (mPGES-2) were found to be overexpressed in colorectal cancer (CRC); however, the role and regulation of these enzymes in this malignancy are not yet fully understood. Here, we report that the cyclopentenone prostaglandins (CyPGs) 15-deoxy-Δ12,14-PGJ2 and PGA2 downregulate mPGES-2 expression in the colorectal carcinoma cell lines Caco-2 and HCT 116 without affecting the expression of any other PGES or COX. Inhibition of mPGES-2 was subsequently followed by decreased microsomal PGES activity. These effects were mediated via modulation of the cellular thiol-disulfide redox status but did not involve activation of the peroxisome proliferator-activated receptor γ or PGD2 receptors. CyPGs had antiproliferative properties in vitro; however, this biological activity could not be directly attributed to decreased PGES activity because it could not be reversed by adding PGE2. Our data suggest that there is a feedback mechanism between PGE2 and CyPGs that implicates mPGES-2 as a new potential target for pharmacological intervention in CRC.
Introduction: Lumbosacral fixation is a common procedure in primary and revision spine surgery but leads to high biomechanical stress on adjacent segments and the SIJ, resulting in implant failure such as breakage and loosening and pain. This frequently results in further surgery. For patients showing clinical and radiological signs of SIJ affection/arthrosis who fail conservative therapy, transarticular lumbopelvic fusion via the SIJ may be considered. The Bedrock™ technique has been described as a new option for reinforced lumbopelvic fixation, fusing the SIJ with additional triangular titanium implants, thereby reducing biomechanical loads off the S2AI screws. We share our experiences with 19 patients treated with this technique since January 2019.
Materials and Methods: 19 patients suffering from persisting low back pain (LBP) with indication for reinforced lumbopelvic fixation and SIJ fusion were treated with reinforced lumboplevic fixation with S2AI screw and a triangular titanium implant. 14 cases were revisions. All surgeries were carried out by a single surgeon at a orthopedic university hospital. Data was gathered retrospectively.
Results: From 1/2019 - 9/2021 19 patients (11f, 8m) were treated with reinforced lumbopelvic fixation and SIJ fusion with a mean follow up of 18,2 months. Mean age 68 years (range 62-78y). Preop. walking distance was reduced to an average <100 m. Standard treatment involved S2AI screws and triangular titanium implants (SIBone, iFuse 3D™). 14 revision cases split into 5 low grade infections with screw loosening, 3 cases with rod breakage, 5 cases of painful lumbopelvic screw prominence, 7 cases with proximal junctional kyphosis, 2 cases with misplaced implants, 8 cases of poor bone mineral density. 5 patients without prior spine surgery. All patients were treated bilaterally using freehand technique. Average implant length was 65 mm. There were no intraoperative or implant associated adverse events (AE) or serious adverse events (SAE). Postoperative imaging demonstrated good implant positioning and function. All patients regained walking ability for distances > 1000 m and were satisfied with the result. All patients reported significant reduction of SIJ pain.
Conclusion: We report results of 19 patients with a reinforced lumbopelvic fixation and fusion by S2AI screws augmented by one parallelly placed triangular titanium implant fusing the SIJ bilaterally with a mean follow-up of 18.2 months. Intra- and postoperatively we experienced no implant associated adverse event. Patients regained significant walking ability and significant reduction of SIJ pain. Radiologically no signs of implant loosening or failure were detected at the end of follow-up. Our results demonstrate a safe and efficacious surgical technique for reinforced lumbopelvic fixation with fusion of SIJ with significant improvement of the health care related quality of life. Further studies need to be conducted in order to obtain additional evidence.
In einer prospektiven Studie wurden 68 Patienten mit Ersatz des vorderen und/oder hinteren Kreuzbands 2, 12, 24 Wochen, 1, 1,5 und 2 Jahre postoperativ kontrastmittelunterstützt magnetresonanztomographisch und klinisch untersucht, insgesamt wurden 160 MR-Untersuchungen durchgeführt. Sagittale, parasagittale und koronale Bilder wurden nativ mittels T1- und T2 gewichteter Spin Echo (SE) und Turbospinecho- (TSE) Sequenzen und nach Kontrastmittelgabe mit T1- und FS-Sequezen akquiriert. Die Resultate wurden mit den klinischen Untersuchungsergebnissen verglichen. Die Kriterien der MR-Auswertung umfassten die Morphologie, Signalintensität, das Kontrastmittelaufnahmeverhalten des Transplantates und Sekundärzeichen und der Vergleich mit den klinischen Untersuchungen einschließlich Stabilitäts- und Funktionstests entsprechend der Scores nach IKDC, OAK und Lysholm. Zwei Wochen postoperativ zeigten alle Kreuzbandtransplantate ein homogenes niedriges Signal in den T1 und T2 gewichteten SE Sequenzen, eine Unterscheidung zu den Signalen normaler Kreuzbänder sowie der verbliebenen Patellarsehne war nicht möglich. Innerhalb des ersten postoperativen Jahres konnte bei allen Patienten eine kontinuierliche Zunahme der Signalintensität und damit Inhomogenität des Bandersatzes beobachtet werden. Die Werte stiegen von 1,1a.u. auf 6,9 a.u. nach einem Jahr. Eine Beurteilung des Kreuzbandersatzes war nicht möglich. Im darauffolgenden postoperativen zweiten Jahr kam es zu einer Normalisierung des Signalintensitätsverhalten des Transplantates und somit zu einer besseren Abgrenzbarkeit (C/N=3,0). Alle Patienten wiesen postoperative einen unkomplizierten Verlauf mit klinisch stabilem Transplantat auf.
The long-chain fatty acid receptor FFAR1 is highly expressed in pancreatic β-cells. Synthetic FFAR1 agonists can be used as antidiabetic drugs to promote glucose-stimulated insulin secretion (GSIS). However, the physiological role of FFAR1 in β-cells remains poorly understood. Here we show that 20-HETE activates FFAR1 and promotes GSIS via FFAR1 with higher potency and efficacy than dietary fatty acids such as palmitic, linoleic, and α-linolenic acid. Murine and human β-cells produce 20-HETE, and the ω-hydroxylase-mediated formation and release of 20-HETE is strongly stimulated by glucose. Pharmacological inhibition of 20-HETE formation and blockade of FFAR1 in islets inhibits GSIS. In islets from type-2 diabetic humans and mice, glucose-stimulated 20-HETE formation and 20-HETE-dependent stimulation of GSIS are strongly reduced. We show that 20-HETE is an FFAR1 agonist, which functions as an autocrine positive feed-forward regulator of GSIS, and that a reduced glucose-induced 20-HETE formation contributes to inefficient GSIS in type-2 diabetes.
Background We published the Canadian 2003 International Consensus Algorithm for the Diagnosis, Therapy, and Management of Hereditary Angioedema (HAE; C1 inhibitor [C1-INH] deficiency) and updated this as Hereditary angioedema: a current state-of-the-art review: Canadian Hungarian 2007 International Consensus Algorithm for the Diagnosis, Therapy, and Management of Hereditary Angioedema. Objective To update the International Consensus Algorithm for the Diagnosis, Therapy and Management of Hereditary Angioedema (circa 2010). Methods The Canadian Hereditary Angioedema Network (CHAEN)/Reseau Canadien d'angioedeme hereditaire (RCAH) (www.haecanada.com) and cosponsors University of Calgary and the Canadian Society of Allergy and Clinical Immunology (with an unrestricted educational grant from CSL Behring) held our third Conference May 15th to 16th, 2010 in Toronto Canada to update our consensus approach. The Consensus document was reviewed at the meeting and then circulated for review. Results This manuscript is the 2010 International Consensus Algorithm for the Diagnosis, Therapy and Management of Hereditary Angioedema that resulted from that conference. Conclusions Consensus approach is only an interim guide to a complex disorder such as HAE and should be replaced as soon as possible with large phase III and IV clinical trials, meta analyses, and using data base registry validation of approaches including quality of life and cost benefit analyses, followed by large head-to-head clinical trials and then evidence-based guidelines and standards for HAE disease management.
Acute calculus cholecystitis is a very common disease with several area of uncertainty. The World Society of Emergency Surgery developed extensive guidelines in order to cover grey areas. The diagnostic criteria, the antimicrobial therapy, the evaluation of associated common bile duct stones, the identification of “high risk” patients, the surgical timing, the type of surgery, and the alternatives to surgery are discussed. Moreover the algorithm is proposed: as soon as diagnosis is made and after the evaluation of choledocholitiasis risk, laparoscopic cholecystectomy should be offered to all patients exception of those with high risk of morbidity or mortality. These Guidelines must be considered as an adjunctive tool for decision but they are not substitute of the clinical judgement for the individual patient.
Ziel der Studie: Die akute alkoholinduzierte Fettleber stellt das erste Stadium alkoholischer Leberer-krankungen dar. Bereits eine akute Alkoholintoxikation führt zu einer signifikanten Ak-kumulation von Fett in den Hepatozyten. Trotz verbesserter serologischer und bildge-bender Diagnoseverfahren ist die Leberbiopsie nach wie vor der Goldstandard zur Di-agnose einer Fettleber. Mögliche Komplikationen als invasives Verfahren, Stichproben-fehler sowie eine geringe Sensitivität im Bereich geringgradiger Verfettung sind die größten Nachteile der Leberbiopsie. Ziel dieser Studie war es, mit Hilfe der 1H-Magnetresonzspektroskopie eine akute alkoholinduzierte Fettleber zu diagnostizieren und quantitativ zu beurteilen. Um die Korrelation zwischen spektroskopisch gemesse-nem Leberfettgehalt und histologischer, biochemischer sowie laborchemischer Analyse zu bestimmen, wurde eine alkoholinduzierte Fettleber im Tiermodell verwendet.
Methodik: In 20 Lewis-Ratten wurde eine alkoholische Fettleber mittels gastraler Ethanol-Applikation induziert; 10 Ratten dienten als Kontrolle. Der intrahepatische Fettgehalt wurde mittels 1H-MRS (3.0 T) als prozentuales Verhältnis zwischen Lipid- und Was-ser-Peak berechnet. Fettgehalt sowie Triglyceride wurden nach Entnahme der Leber histologisch und biochemisch (nach FOLCH) bestimmt. Um spezifische Leberenzyme zu untersuchen, wurde Blut aus dem orbitalen Venenplexus entnommen.
Ergebnisse: In allen 20 Tieren konnte nach Ethanolapplikation eine Leberverfettung mittels 1H-MRS nachgewiesen werden. Histologisch zeigten 16 Tiere eine Fettleber. Ebenso zeigte sich nach Ethanolgabe und folgender biochemischer Analyse im Durschnitt eine Erhö-hung des Triglyceridgehalts, welcher einer Leberverfettung entsprach. Es fanden sich statistisch signifikante Korrelationen zwischen der histologisch bestimmten intrahepati-schen Verfettung und dem spektroskopisch gemessenen Fettgehalt (Pearson-Korrelationskoeffizient r = 0.90, p < 0.01) sowie zwischen der biochemischen Analyse nach FOLCH und 1H-MRS (r = 0.97, p < 0.01). Ebenso zeigte sich eine positive signifi-kante Korrelation zwischen spektroskopisch gemessener Leberverfettung und den Le-berparametern AST (r = 0.91, p < 0.05) und ALT (r = 0.84, p < 0.05).
Schlussfolgerung: Es konnte gezeigt werden, dass bereits geringgradige intrahepatische Verfettungen prä-zise quantitativ mittels 3.0 T-Protonen-MR-Spektroskopie darstellbar sind. Somit kann die Methode – bei Beachtung geeigneter Grenzwerte histologischer Messungen – als verlässliche diagnostische Alternative in Betracht gezogen werden. Bereits 48 Stunden nach Alkoholintoxikation ist mittels 1H-MRS eine exakte Differenzierung gesunder und pathologischer Lebern möglich – entscheidend vor allem im Rahmen einer Prätrans-plantationsdiagnostik bei Verdacht auf eine akute Fettleber. Darüber hinaus ist die 1H-MRS anderen bildgebenden Verfahren wie Ultraschall, CT und MRT in der genauen Quantifizierung intrahepatischen Fettgehalts überlegen. Der Einsatz erscheint auch in der Diagnostik und Verlaufskontrolle alkoholischer Lebererkrankungen in Zukunft sinnvoll. Kommende Untersuchung sollten an einem größeren Probandenkollektiv durchgeführt werden, um den Stellewert der 1H-MRS zu unterstreichen.
The Nodular lymphocyte-predominant Hodgkin lymphoma (NLPHL) as well as the T-cell/histiocyte-rich large B-cell lymphoma (THRLBCL) are rare types of malignant lymphomas. Both NLPHL and THRLBCL are frequently observed in middle-aged men with THRLBCL presenting frequently with an advanced Ann-Arbor stage with B-symptoms and associated with more aggressive courses.3 However, due to the limited number of tumor cells in the tissue of both NLPHL and THRLBCL, limited numbers of studies have been conducted on these lymphomas and current results are mainly based on general molecular genetic studies.
In order to obtain a better understanding for these disease forms as well as possible changes in their nuclear and cytoplasmatic sizes, the following study relied on the comparison of the different NLPHL forms and THRLBCL in terms of nuclear size and nuclear volume. This was carried out using both 2D and 3D analysis. During the 2D analysis of nuclear size and nuclear volume no significant differences could be presented between those groups. However, the 3D analysis of NLPHL and THRLBCL pointed out a slightly enlarged nuclear volume in THRLBCL. Furthermore, the analysis indicated a significantly increased cytoplasmatic size of THRLBCL compared to NLPHL forms. Nevertheless, differences occurred not only between the tumor cells of both disease forms, but also the T cells presented a larger nuclear volume in THRLBCL. B cells, which were considered as the control group, did not demonstrate any significant differences between the different groups. The presented results suggest an increased activity of T cells in THRLBCL, which is most likely to be interpreted as a response against the surrounding tumor cells and probably limits the proliferation of the tumor cells. Based on these results, the importance of 3D analysis is also evident due to the fact that it is clearly superior to 2D analysis. For a better understanding of both disease forms, it is therefore recommended to use the 3D technique in combination with molecular genetic analysis in future research.
Nodular lymphocyte-predominant Hodgkin lymphoma (NLPHL) can show variable histological growth patterns and present remarkable overlap with T-cell/histiocyte-rich large B-cell lymphoma (THRLBCL). Previous studies suggest that NLPHL histological variants represent progression forms of NLPHL and THRLBCL transformation in aggressive disease. Since molecular studies of both lymphomas are limited due to the low number of tumor cells, the present study aimed to learn if a better understanding of these lymphomas is possible via detailed measurements of nuclear and cell size features in 2D and 3D sections. Whereas no significant differences were visible in 2D analyses, a slightly increased nuclear volume and a significantly enlarged cell size were noted in 3D measurements of the tumor cells of THRLBCL in comparison to typical NLPHL cases. Interestingly, not only was the size of the tumor cells increased in THRLBCL but also the nuclear volume of concomitant T cells in the reactive infiltrate when compared with typical NLPHL. Particularly CD8+ T cells had frequent contacts to tumor cells of THRLBCL. However, the nuclear volume of B cells was comparable in all cases. These results clearly demonstrate that 3D tissue analyses are superior to conventional 2D analyses of histological sections. Furthermore, the results point to a strong activation of T cells in THRLBCL, representing a cytotoxic response against the tumor cells with unclear effectiveness, resulting in enhanced swelling of the tumor cell bodies and limiting proliferative potential. Further molecular studies combining 3D tissue analyses and molecular data will help to gain profound insight into these ill-defined cellular processes.
Aims: The examination of histological sections is still the gold standard in diagnostic pathology. Important histopathological diagnostic criteria are nuclear shapes and chromatin distribution as well as nucleus-cytoplasm relation and immunohistochemical properties of surface and intracellular proteins. The aim of this investigation was to evaluate the benefits and drawbacks of three-dimensional imaging of CD30+ cells in classical Hodgkin Lymphoma (cHL) in comparison to CD30+ lymphoid cells in reactive lymphoid tissues.
Materials and results: Using immunoflourescence confocal microscopy and computer-based analysis, we compared CD30+ neoplastic cells in Nodular Sclerosis cHL (NScCHL), Mixed Cellularity cHL (MCcHL), with reactive CD30+ cells in Adenoids (AD) and Lymphadenitis (LAD). We confirmed that the percentage of CD30+ cell volume can be calculated. The amount in lymphadenitis was approx. 1.5%, in adenoids around 2%, in MCcHL up to 4,5% whereas the values for NScHL rose to more than 8% of the total cell cytoplasm. In addition, CD30+ tumour cells (HRS-cells) in cHL had larger volumes, and more protrusions compared to CD30+ reactive cells. Furthermore, the formation of large cell networks turned out to be a typical characteristic of NScHL.
Conclusion: In contrast to 2D histology, 3D laser scanning offers a visualisation of complete cells, their network interaction and spatial distribution in the tissue. The possibility to differentiate cells in regards to volume, surface, shape, and cluster formation enables a new view on further diagnostic and biological questions. 3D includes an increased amount of information as a basis of bioinformatical calculations.
Rationale: Classic histology is the gold standard for vascular network imaging and analysis. The method however is laborious and prone to artefacts. Here, the suitability of ultramicroscopy (UM) and micro-computed tomography (CT) was studied to establish potential alternatives to histology.
Methods: The vasculature of murine organs (kidney, heart and atherosclerotic carotid arteries) was visualized using conventional 2D microscopy, 3D light sheet ultramicroscopy (UM) and micro-CT. Moreover, spheroid-based human endothelial cell vessel formation in mice was quantified. Fluorescently labeled Isolectin GS-IB4 A647 was used for in vivo labeling of vasculature for UM analysis, and analyses were performed ex vivo after sample preparation. For CT imaging, animals were perfused postmortem with radiopaque contrast agent.
Results: Using UM imaging, 3D vascular network information could be obtained in samples of animals receiving in vivo injection of the fluorescently labeled Isolectin GS-IB4. Resolution was sufficient to measure single endothelial cell integration into capillaries in the spheroid-based matrigel plug assay. Because of the selective staining of the endothelium, imaging of larger vessels yielded less favorable results. Using micro-CT or even nano-CT, imaging of capillaries was impossible due to insufficient X-ray absorption and thus insufficient signal-to-noise ratio. Identification of lumen in murine arteries using micro-CT was in contrast superior to UM.
Conclusion: UM and micro-CT are two complementary techniques. Whereas UM is ideal for imaging and especially quantifying capillary networks and arterioles, larger vascular structures are easier and faster to quantify and visualize using micro-CT. 3D information of both techniques is superior to 2D histology. UM and micro-CT together may open a new field of clinical pathology diagnosis.
This study deals with 3D laser investigation on the border between the human lymph node T-zone and germinal centre. Only a few T-cells specific for antigen selected B-cells are allowed to enter germinal centres. This selection process is guided by sinus structures, chemokine gradients and inherent motility of the lymphoid cells. We measured gaps and wall-like structures manually, using IMARIS, a 3D image software for analysis and interpretation of microscopy datasets. In this paper, we describe alpha-actin positive and semipermeable walls and wall-like structures that may hinder T-cells and other cell types from entering germinal centres. Some clearly defined holes or gaps probably regulate lymphoid traffic between T- and B-cell areas. In lymphadenitis, the morphology of this border structure is clearly defined. However, in case of malignant lymphoma, the wall-like structure is disrupted. This has been demonstrated exemplarily in case of angioimmunoblastic T-cell lymphoma. We revealed significant differences of lengths of the wall-like structures in angioimmunoblastic T-cell lymphoma in comparison with wall-like structures in reactive tissue slices. The alterations of morphological structures lead to abnormal and less controlled T- and B-cell distributions probably preventing the immune defence against tumour cells and infectious agents by dysregulating immune homeostasis.
Introduction: The treatment of carious lesions is one of the most fundamental competencies in daily dental practice. However, many commercially available training models lack in reality regarding the simulation of pathologies such as carious lesions. 3D printed models could provide a more realistic simulation. This study provides an exemplary description of the fabrication of 3D printed dental models with carious lesions and assesses their educational value compared to commercially available models in conservative dentistry.
Materials and Methods: A single-stage, controlled cohort study was conducted within the context of a curricular course. A stereolithographic model was obtained from an intraoral scan and then printed using fused deposition modelling. These models were first piloted by experts and then implemented and compared against commercial models in a conservative dentistry course. Experts and students evaluated both models using a validated questionnaire. Additionally, a cost analysis for both models was carried out.
Results: Thirteen dentists and twenty-seven 5th year dental students participated in the study. The 3D printed models were rated significantly more realistic in many test areas. In particular, the different tactility and the distinction in colour was rated positively in the 3D printed models. At 28.29€ (compared to 112.36€), the 3D printed models were exceptionally cost-efficient.
Conclusions: 3D printed dental models present a more realistic and cost-efficient alternative to commercial models in the undergraduate training of conservative dentistry.
Popular media now often present 3D printing as a widely employed technology for the production of dental prostheses. This article aims to show, based on factual information, to what extent 3D printing can be used in dental laboratories and dental practices at present. It attempts to present a rational evaluation of todays´ applications of 3D printing technology in the context of dental restorations. In addition, the article discusses future perspectives and examines the ongoing viability of traditional dental laboratory services and manufacturing processes. It also shows which expertise is needed for the digital additive manufacturing of dental restorations.
Mathematical models of virus dynamics have not previously acknowledged spatial resolution at the intracellular level despite substantial arguments that favor the consideration of intracellular spatial dependence. The replication of the hepatitis C virus (HCV) viral RNA (vRNA) occurs within special replication complexes formed from membranes derived from endoplasmatic reticulum (ER). These regions, termed membranous webs, are generated primarily through specific interactions between nonstructural virus-encoded proteins (NSPs) and host cellular factors. The NSPs are responsible for the replication of the vRNA and their movement is restricted to the ER surface. Therefore, in this study we developed fully spatio-temporal resolved models of the vRNA replication cycle of HCV. Our simulations are performed upon realistic reconstructed cell structures—namely the ER surface and the membranous webs—based on data derived from immunostained cells replicating HCV vRNA. We visualized 3D simulations that reproduced dynamics resulting from interplay of the different components of our models (vRNA, NSPs, and a host factor), and we present an evaluation of the concentrations for the components within different regions of the cell. Thus far, our model is restricted to an internal portion of a hepatocyte and is qualitative more than quantitative. For a quantitative adaption to complete cells, various additional parameters will have to be determined through further in vitro cell biology experiments, which can be stimulated by the results deccribed in the present study.
In Bone Tissue Engineering (BTE), autologous bone-regenerative cells are combined with a scaffold for large bone defect treatment (LBDT). Microporous, polylactic acid (PLA) scaffolds showed good healing results in small animals. However, transfer to large animal models is not easily achieved simply by upscaling the design. Increasing diffusion distances have a negative impact on cell survival and nutrition supply, leading to cell death and ultimately implant failure. Here, a novel scaffold architecture was designed to meet all requirements for an advanced bone substitute. Biofunctional, porous subunits in a load-bearing, compression-resistant frame structure characterize this approach. An open, macro- and microporous internal architecture (100 µm–2 mm pores) optimizes conditions for oxygen and nutrient supply to the implant’s inner areas by diffusion. A prototype was 3D-printed applying Fused Filament Fabrication using PLA. After incubation with Saos-2 (Sarcoma osteogenic) cells for 14 days, cell morphology, cell distribution, cell survival (fluorescence microscopy and LDH-based cytotoxicity assay), metabolic activity (MTT test), and osteogenic gene expression were determined. The adherent cells showed colonization properties, proliferation potential, and osteogenic differentiation. The innovative design, with its porous structure, is a promising matrix for cell settlement and proliferation. The modular design allows easy upscaling and offers a solution for LBDT.
Background: Recent advances in 3D printing technology have enabled the emergence of new educational and clinical tools for medical professionals. This study provides an exemplary description of the fabrication of 3D‐printed individualised patient models and assesses their educational value compared to cadaveric models in oral and maxillofacial surgery.
Methods: A single‐stage, controlled cohort study was conducted within the context of a curricular course. A patient's CT scan was segmented into a stereolithographic model and then printed using a fused filament 3D printer. These individualised patient models were implemented and compared against cadaveric models in a curricular oral surgery hands‐on course. Students evaluated both models using a validated questionnaire. Additionally, a cost analysis for both models was carried out. P‐values were calculated using the Mann‐Whitney U test.
Results: Thirty‐eight fourth‐year dental students participated in the study. Overall, significant differences between the two models were found in the student assessment. Whilst the cadaveric models achieved better results in the haptic feedback of the soft tissue, the 3D‐printed individualised patient models were regarded significantly more realistic with regard to the anatomical correctness, the degree of freedom of movement and the operative simulation. At 3.46 € (compared to 6.51 €), the 3D‐printed patient individualised models were exceptionally cost‐efficient.
Conclusions: 3D‐printed patient individualised models presented a realistic alternative to cadaveric models in the undergraduate training of operational skills in oral and maxillofacial surgery. Whilst the 3D‐printed individualised patient models received positive feedback from students, some aspects of the model leave room for improvement.
Purpose of the study: There is a clinical need for antiretroviral therapy (ART) regimens that simplify dosing and make adherence easier for specific patient groups such as former intravenous drug users (IVDU) receiving opiate substitution. Availability of tenofovir DF (TDF) and other once-daily (OD) agents could offer a viable OD regimen. The 3OD study was designed to evaluate the use of OD HAART in IVDU patients.
Methods: 3OD was a single-arm, multicentre, 48-weeks trial to assess efficacy, tolerability and adherence to a OD TDF-containing HAART regimen in former IVDU patients receiving opiate substitution. Of 67 patients enrolled, 27 were antiretroviral treatment naïve, 10 were virologically suppressed (<400 copies/mL), and 30 were re-starting HAART without prior virological failure. Opiate substitution was adjusted according to subject symptoms of opiate overdosing or withdrawal. Various methods were used to assess adherence: besides pill count, patients were asked to fill in a MASRI (Medication Adherence Self-Report Inventory) questionnaire and an electronic log pad diary. Calculation of adherence by pill count assumed that unreturned pills had been taken by the subjects.
Summary of results: Overall, 55% (n = 37, ITT, M = F) of patients had viral load <400 copies/mL at week 48. Using an ITT, M = E analysis, 90% (37/41) of patients reached undetectable VL (<400 copies/mL), 56% (23/41 patients) had plasma HIV-1 RNA concentrations <50 copies/mL at week 48. Only 30 patients (45%) completed the full study and the follow-up period. In 51% of patients, TDF adherence was >100% using pill count. MASRI showed adherence rates of 80–100% in 83–85% of patients; however, 15 patients never entered any data. Diary data were entered by 57 patients; diary data were entered for fewer days than patients received treatment (mean difference 113 days, calculated from treatment start and stop dates).
Conclusion: TDF in combination with other OD antiretrovirals in former IVDU patients showed comparable efficacy to that seen in the average HIV-1 infected population. However, measurement of adherence to self-administered HAART via pill count, MASRI or diary may be misleading in this population.
Murine acetaminophen-induced acute liver injury (ALI) serves as paradigmatic model for drug-induced hepatic injury and regeneration. As major cause of ALI, acetaminophen overdosing is a persistent therapeutic challenge with N-acetylcysteine clinically used to ameliorate parenchymal necrosis. To identify further treatment strategies that serve patients with poor N-acetylcysteine responses, hepatic 3′mRNA sequencing was performed in the initial resolution phase at 24 h/48 h after sublethal overdosing. This approach disclosed 45 genes upregulated (≥5-fold) within this time frame. Focusing on C5aR1, we observed in C5aR1-deficient mice disease aggravation during resolution of intoxication as evidenced by increased liver necrosis and serum alanine aminotransferase. Moreover, decreased hepatocyte compensatory proliferation and increased caspase-3 activation at the surroundings of necrotic cores were detectable in C5aR1-deficient mice. Using a non-hypothesis-driven approach, herein pro-regenerative/-resolving effects of C5aR1 were identified during late acetaminophen-induced ALI. Data concur with protection by the C5a/C5aR1-axis during hepatectomy and emphasize the complex role of inflammation during hepatic regeneration and repair.
Patient therapy is based mainly on a combination of diagnosis, suitable monitoring or support devices and drug treatment and is usually employed for a pre-existing disease condition. Therapy remains predominantly symptom-based, although it is increasingly clear that individual treatment is possible and beneficial. However, reasonable precision medicine can only be realized with the coordinated use of diagnostics, devices and drugs in combination with extensive databases (4Ds), an approach that has not yet found sufficient implementation. The practical combination of 4Ds in health care is progressing, but several obstacles still hamper their extended use in precision medicine.
Osteosynthesematerialien aus resorbierbaren Materialien finden aufgrund ihrer zunehmenden biomechanischen Stabilität eine immer höhere Akzeptanz. Die vorgelegte Untersuchung vergleicht die knöcherne und okklusale Stabilität von solchen Osteosynthesesystemen aus resorbierbaren Materialien im Vergleich mit konventionellen Titanminiplatten in der orthognathen Chirurgie. Untersucht wurden 100 Patienten in einem Nachsorgezeitraum von bis zu 4 Jahren. Die Beurteilung erfolgte klinisch und röntgenologisch anhand von Fernröntgen-Seiten-Aufnahmen. In die Studie gingen ein fünfzig Patienten, die mit resorbierbaren Platten versorgt worden waren, 16 davon mit Polylactid-Polyglycolid Copolymer Platten und 34 Patienten mit 70:30 Poly-L/DL-Lactid Copolymer Platten; fünfzig weitere Patienten, versorgt mit einem Titanminiplattensystem wurden als Kontrollgruppe gewertet. Zunächst konnte gezeigt werden, dass die Gruppen zueinander homogen waren, es unterschieden sich die effektiven intraoperativen Bewegungen von Studien und Kontrollgruppe statistisch nicht signifikant, gleiches galt im Vergleich der resorbierbaren Platten untereinander. Bei der Datenauswertung konnte nachgewiesen werden, dass die absolute und relative Instabilität weder zwischen den Studiengruppen noch der Kontrollgruppe noch zwischen den resorbierbaren Plattensystemen untereinander signifikant unterschiedlich war. Die postoperative 1-Jahres Stabilität unterschied sich dabei ebenfalls nicht signifikant von der 4-Jahres Stabilität. Alle untersuchten Osteosynthesesysteme zeigten im Oberkiefer eine höhere Stabilität als im Unterkiefer und gleichzeitig eine größere Stabilität in der anterior-posterioren Richtung als in der inferior-superioren Richtung. Es wurden keine signifikanten Unterschiede zwischen der klinischen Stabilität von PLGA oder P(L/DL)LA-Miniplatten gefunden. Allerdings konnte auch belegt werden, dass der Einsatz resorbierbarer Unterkieferosteosynthesen eine sehr gute Compliance des Patienten bezüglich geführter Okklusion und Nachsorge voraussetzte. Als nachteilig bei den resorbierbaren Systemen konnte sowohl für den Oberkiefer als auch für den Unterkiefer eine leicht erhöhte Mobilität bis zu 6 Wochen nach der Operation festgestellt werden, die jedoch in keinem Fall das operative Ergebnis beeinflusste. Vielmehr ermöglichten resorbierbare Osteosynthesen eine bessere postoperative okklusale Einstellung. Weiteren Nachteil der resorbierbaren Osteosynthesesysteme bedeuten die hohen Kosten, die heute noch weit über denen von Titanminiplatten liegen. Zukünftige Entwicklungen gehen dahin, das Plattendesign weiter zu optimieren, wie z.B. neuartige pinbasierte Fixationssysteme einzusetzen, wie sie in der Extremitätenchirurgie teils bereits benutzt werden. Fernziel für die Zukunft ist die Entwicklung eines resorbierbaren Osteosynthesesystems, das sich bei geringen Herstellungskosten und miniaturisiertem Plattendesign auch minimalinvasiv einbringen lässt.
The stimulation of the AMP-activated kinase (AMPK) by 5-amino-1-β-D-ribofuranosyl-imidazole-4-carboxamide (AICAR) has been associated with antihyperalgesia and the inhibition of nociceptive signaling in the spinal cord in models of paw inflammation. The attenuated nociception comes along with a strongly reduced paw edema, indicating that peripheral antiinflammatory mechanisms contribute to antinociception. In this study, we investigated the impact of AICAR on the immune cell composition in inflamed paws, as well as the regulation of inflammatory and resolving markers in macrophages. By using fluorescence-activated cell sorting (FACS) analysis and immunofluorescence, we found a significantly increased fraction of proresolving M2 macrophages and anti-inflammatory interleukin (IL)-10 in inflamed tissue, while M1 macrophages and proinflammatory cytokines such as IL-1 were decreased by AICAR in wild type mice. In AMPKα2 knock-out mice, the M2 polarization of macrophages in the paw was missing. The results were supported by experiments in primary macrophage cultures which also showed a shift to a proresolving phenotype with decreased levels of proinflammatory mediators and increased levels of antiinflammatory mediators. However, in the cell cultures, we did not observe differences between the AMPKα2+/+ and −/− cells, thus indicating that the AICAR-induced effects are at least partially AMPK-independent. In summary, our results indicate that AICAR has potent antiinflammatory and proresolving properties in inflammation which are contributing to a reduction of inflammatory edema and antinociception.
5-iodotubercidin sensitizes cells to RIPK1-dependent necroptosis by interfering with NFκB signaling

Receptor-interacting protein kinases (RIPK) −1 and −3 are master regulators of cell fate decisions in response to diverse stimuli and are subjected to multiple checkpoint controls. Earlier studies have established the presence of distinct IKK1/2 and p38/MK2-dependent checkpoints which suppress RIPK1 activation by directly phosphorylating it at different residues. In the present study, we investigated TNF-induced death in MAPK-activated protein kinase 2 (MK2)-deficient cells and show that MK2-deficiency or inactivation predominantly results in necroptotic cell death, even in the absence of caspase inhibition. While MK2-deficient cells can be rescued from necroptosis by RIPK1 inhibitors, RIPK3 inhibition seems to revert the process triggering apoptosis. To understand the mechanism of this necroptosis switch, we screened a 149-compound kinase inhibitor library for compounds which preferentially sensitize MK2-deficient MEFs to TNF-induced cell death. The most potent inhibitor identified was 5-Iodotubericidin, an adenosine analogue acting as adenosine kinase and protein kinase inhibitor. 5-ITu also potentiated LPS-induced necroptosis when combined with MK2 inhibition in RAW264.7 macrophages. Further mechanistic studies revealed that 5-Iodotubericidin induces RIPK1-dependent necroptosis in the absence of MK2 activity by suppressing IKK signaling. The identification of this role for the multitarget kinase inhibitor 5-ITu in TNF-, LPS- and chemotherapeutics-induced necroptosis will have potential implications in RIPK1-targeted therapies.
5-Lipoxygenase contributes to PPAR [gamma] activation in macrophages in response to apoptotic cells

Background: One hallmark contributing to immune suppression during the late phase of sepsis is macrophage polarization to an anti-inflammatory phenotype upon contact with apoptotic cells (AC). Taking the important role of the nuclear receptor PPARγ for this phenotype switch into consideration, it remains elusive how AC activate PPARγ in macrophages. Therefore, we were interested to characterize the underlying principle.
Methods: Apoptosis was induced by treatment of Jurkat T cells for 3 hours with 0.5 μg/ml staurosporine. Necrotic cells (NC) were prepared by heating cells for 20 minutes to 65°C. PPARγ activation was followed by stably transducing RAW264.7 macrophages with a vector encoding the red fluorescent protein mRuby after PPARγ binding to 4 × PPRE sites downstream of the reporter gene sequence. This readout was established by treatment with the PPARγ agonist rosiglitazone (1 μM) and AC (5:1). Twenty-four hours after stimulation, mRuby expression was analysed by fluorescence microscopy. Lipid rafts of AC, NC, as well as living cells (LC) were enriched by sucrose gradient centrifugation. Fractions were analysed for lipid raft-associated marker proteins. Lipid rafts were incubated with transduced RAW264.7 macrophages as described above. 5-Lipoxygenase (5-LO) involvement was verified by pharmacological inhibition (MK-866, 1 μM) and overexpression.
Results: Assuming that the molecule responsible for PPARγ activation in macrophages is localized in the cell membrane of AC, most probably associated to lipid rafts, we isolated lipid rafts from AC, NC and LC. Mass spectrometric analysis of lipid rafts of AC showed the expression of 5-LO, whereas lipid rafts of LC did not. Moreover, incubating macrophages with lipid rafts of AC induced mRuby expression. In contrast, lipid rafts of NC and LC did not. To verify the involvement of 5-LO in activating PPARγ in macrophages, Jurkat T cells were incubated for 30 minutes with the 5-LO inhibitor MK-866 (1 μM) before apoptosis induction. In line with our hypothesis, these AC did not induce mRuby expression. Finally, although living Jurkat T cells overexpressing 5-LO did not activate PPARγ in macrophages, mRuby expression was significantly increased when AC were generated from 5-LO overexpressing compared with wild-type Jurkat cells.
Conclusion: Our results suggest that induction of apoptosis activates 5-LO, localizing to lipid rafts, necessary for PPARγ activation in macrophages. Therefore, it will be challenging to determine whether 5-LO activity in AC, generated from other cell types, correlates with PPARγ activation, contributing to an immune-suppressed phenotype in macrophages.
Leukotrienes constitute a group of bioactive lipids generated by the 5-lipoxygenase (5-LO) pathway. An increasing body of evidence supports an acute role for 5-LO products already during the earliest stages of pancreatic, prostate, and colorectal carcinogenesis. Several pieces of experimental data form the basis for this hypothesis and suggest a correlation between 5-LO expression and tumor cell viability. First, several independent studies documented an overexpression of 5-LO in primary tumor cells as well as in established cancer cell lines. Second, addition of 5-LO products to cultured tumor cells also led to increased cell proliferation and activation of anti-apoptotic signaling pathways. 5-LO antisense technology approaches demonstrated impaired tumor cell growth due to reduction of 5-LO expression. Lastly, pharmacological inhibition of 5-LO potently suppressed tumor cell growth by inducing cell cycle arrest and triggering cell death via the intrinsic apoptotic pathway. However, the documented strong cytotoxic off-target effects of 5-LO inhibitors, in combination with the relatively high concentrations of 5-LO products needed to achieve mitogenic effects in cell culture assays, raise concern over the assignment of the cause, and question the relationship between 5-LO products and tumorigenesis. Background and Aims: The Roadmap concept is a therapeutic framework in chronic hepatitis B for the intensification of nucleoside analogue monotherapy based on early virologic response. The efficacy and safety of this approach applied to telbivudine treatment has not been investigated.
Methods: A multinational, phase IV, single-arm open-label study (ClinicalTrials.gov ID NCT00651209) was undertaken in HBeAg-positive, nucleoside-naive adult patients with chronic hepatitis B. Patients received telbivudine (600 mg once-daily) for 24 weeks, after which those with undetectable serum HBV DNA (<300 copies/mL) continued to receive telbivudine alone while those with detectable DNA received telbivudine plus tenofovir (300 mg once-daily). Outcomes were assessed at Week 52.
Results: 105 patients commenced telbivudine monotherapy, of whom 100 were included in the efficacy analysis. Fifty-five (55%) had undetectable HBV DNA at Week 24 and continued telbivudine monotherapy; 45 (45%) received tenofovir intensification. At Week 52, the overall proportion of undetectable HBV DNA was 93% (93/100) by last-observation-carried-forward analysis (100% monotherapy group, 84% intensification group) and no virologic breakthroughs had occurred. ALT normalization occurred in 77% (87% monotherapy, 64% intensification), HBeAg clearance in 43% (65% monotherapy, 16% intensification), and HBeAg seroconversion in 39% (62% monotherapy, 11% intensification). Six patients had HBsAg clearance. Myalgia was more common in the monotherapy group (19% versus 7%). No decrease in the mean glomerular filtration rate occurred in either treatment group at Week 52.
Conclusions: Telbivudine therapy with tenofovir intensification at Week 24, where indicated by the Roadmap strategy, appears effective and well tolerated for the treatment of chronic hepatitis B.

Only a few Methyl-[11C]-l-methionine (MET) positron emission tomography (PET) studies have focused on children and young adults with brain neoplasm. Due to radiation exposure, long scan acquisition time, and the need for sedation in young children MET-PET studies should be restricted to this group of patients when a decision for further therapy is not possible from routine diagnostic procedures alone, e.g., structural imaging. We investigated the diagnostic accuracy of MET-PET for the differentiation between tumorous and non-tumorous lesions in this group of patients. Forty eight MET-PET scans from 39 patients aged from 2 to 21 years (mean 15 ± 5.0 years) were analyzed. The MET tumor-uptake relative to a corresponding control region was calculated. A receiver operating characteristic (ROC) was performed to determine the MET-uptake value that best distinguishes tumorous from non-tumorous brain lesions. A differentiation between tumorous (n = 39) and non-tumorous brain lesions (n = 9) was possible at a threshold of 1.48 of relative MET-uptake with a sensitivity of 83% and a specificity of 92%, respectively. A differentiation between high grade malignant lesions (mean MET-uptake = 2.00 ± 0.46) and low grade tumors (mean MET-uptake = 1.84 ± 0.31) was not possible. There was a significant difference in MET-uptake between the histologically homogeneous subgroups of astrocytoma WHO grade II and anaplastic astrocytoma WHO grade III (P = 0.02). MET-PET might be a useful tool to differentiate tumorous from non-tumorous lesions in children and young adults when a decision for further therapy is difficult or impossible from routine structural imaging procedures alone.
