The process 𝑒+𝑒−→Σ+¯Σ− is studied from threshold up to 3.04 GeV/𝑐2 via the initial-state radiation technique using data with an integrated luminosity of 12.0 fb−1, collected at center-of-mass energies between 3.773 and 4.258 GeV with the BESIII detector at the BEPCII collider. The pair production cross sections and the effective form factors of Σ are measured in eleven Σ+¯Σ− invariant mass intervals from threshold to 3.04 GeV/𝑐2. The results are consistent with the previous results from Belle and BESIII. Furthermore, the branching fractions of the decays 𝐽/𝜓→Σ+¯Σ− and 𝜓(3686)→Σ+¯Σ− are determined and the obtained results are consistent with the previous results of BESIII.
Das Fach "Allgemeine und vergleichende Literaturwissenschaft (AVL)" beschäftigt sich mit dem Vergleich von Literatur aus verschiedenen Kulturen, Epochen und Gattungen. Der Begriff "Komparatistik" wird weitgehend synonym verwendet, legt aber einen noch stärkeren Schwerpunkt auf den Vergleich als allgemeine epistemologische Praxis, vor dem der Fokus auf Literatur in den Hintergrund geraten kann. Die Geschichte des Fachs in Deutschland reicht bis in das 19. Jahrhundert zurück, in dem es - in etwa zeitgleich mit den "Nationalphilologien" (hier also Germanistik) - vor allem im Sinne einer vergleichenden Literaturgeschichte entstand. Während in den Anfängen Einflussforschung und Gattungsgeschichte im Vordergrund standen, hat das Fach im 20. Jahrhundert mehrere Ausweitungen erfahren. So zählen beispielsweise auch die allgemeine Literatur- und Kulturtheorie, die Ästhetik und der Vergleich von Literatur mit anderen Medien zu den Forschungsgebieten der AVL. Schon früh wurde der grenzüberschreitende Anspruch der AVL über den von J.W. Goethe lancierten Begriff "Weltliteratur" bestimmt. Entsprechend gehört auch die Frage, wie der Begriff historisch verstanden wurde und wie er im Zeitalter beschleunigter Globalisierung zu denken ist, zu den prominentesten Debatten im Rahmen der AVL. In der ursprünglichen Konzeption von Goethe war "Weltliteratur" als Dialog zwischen den Nationalliteraturen gedacht, wobei sowohl Rezeption (des kulturell Fremden) als auch Widerspiegelung (des Eigenen in der Rezeption durch Andere) im Vordergrund standen. Heute wird die eurozentrische Orientierung des Begriffs problematisiert, aber die Frage bleibt virulent, ob und wie Literatur aus verschiedenen Kulturen und Sprachen auf eine universelle Ebene gehoben werden kann, ohne ihre spezifischen Kontexte zu verlieren.
Als Carlo Ginzburg die These formulierte, dass die Geisteswissenschaften wie die Kriminalliteratur im sog. "Indizienparadigma" gründeten, hatte er mit Sherlock Holmes einen Detektiv vor Augen, der persönlich den Tatort besichtigte. Dort erhob er Spuren, kombinierte sie und kam in oftmals ingeniösen, aber auch höchst spekulativen Schlussfolgerungen zur Lösung seines Falls. Vor dem Hintergrund aktueller Entwicklungen in Forschung und Fahndung muss dieses materiell und empirisch grundierte "Indizienparadigma" jedoch einer Revision unterzogen werden. Denn seit der Privatdetektiv von "Kommissar Computer" Konkurrenz bekommen hat, haben sich die Investigationspraktiken grundlegend gewandelt: So können computergestützte Fahndungs- und Aufklärungsmethoden eine Besichtigung des Tatorts ersetzen, während algorithmische Wahrscheinlichkeitsrechnung vergangene wie zukünftige Fälle erhellt. Der vorliegende Sammelband mit Beiträgen aus der Literatur-, Medien- und Designwissenschaft untersucht, inwiefern solche "virtuellen Investigationen" in Literatur und Kunst der Gegenwart eine Revision des Indizienparadigmas einschließen - und inwiefern Begriffe der Virtualität bereits die Investigativarbeit im 19. Jahrhundert prägten.
This short paper gives a brief overview of the manifestly covariant canonical gauge gravity (CCGG) that is rooted in the De Donder-Weyl Hamiltonian formulation of relativistic field theories, and the proven methodology of the canonical transformation theory. That framework derives, from a few basic physical and mathematical assumptions, equations describing generic matter and gravity dynamics with the spin connection emerging as a Yang Mills-type gauge field. While the interaction of any matter field with spacetime is fixed just by the transformation property of that field, a concrete gravity ansatz is introduced by the choice of the free (kinetic) gravity Hamiltonian. The key elements of this approach are discussed and its implications for particle dynamics and cosmology are presented. New insights: Anomalous Pauli coupling of spinors to curvature and torsion of spacetime, spacetime with (A)dS ground state, inertia, torsion and geometrical vacuum energy, Zero-energy balance of the Universe leading to a vanishing cosmological constant and torsional dark energy.
In heavy-ion collision experiments, the global collectivity of final-state particles can be quantified by anisotropic flow coefficients (vn). The first-order flow coefficient, also referred to as the directed flow (v1), describes the collective sideward motion of produced particles and nuclear fragments in heavy-ion collisions. It carries information on the very early stage of the collision, especially at large pseudorapidity (η), where it is believed to be generated during the nuclear passage time. Directed flow therefore probes the onset of bulk collective dynamics during thermalization, providing valuable experimental guidance to models of the pre-equilibrium stage. In 2018, the Event Plane Detector (EPD) was installed in STAR and used for the Beam Energy Scan phase-II (BES-II) data taking. The combination of EPD (2.1<|η|<5.1) and high-statistics BES-II data enables us to extend the v1 measurement to the forward and backward η regions. In this paper, we present the measurement of v1 over a wide η range in Au+Au collisions at √sNN= 19.6 and 27 GeV using the STAR EPD. The results of the analysis at √sNN=19.6 GeV exhibit excellent consistency with the previous PHOBOS measurement, while elevating the precision of the overall measurement. The increased precision of the measurement also revealed finer structures in heavy-ion collisions, including a potential observation of the first-order event-plane decorrelation. Multiple physics models were compared to the experimental results. Only a transport model and a three-fluid hybrid model can reproduce a sizable v1 at large η as was observed experimentally. The model comparison also indicates v1 at large η might be sensitive to the QGP phase transition.
In a Quark-Gluon Plasma (QGP), the fundamental building blocks of matter, quarks and gluons, are under extreme conditions of temperature and density. A QGP could exist in the early stages of the Universe, and in various objects and events in the cosmos. The thermodynamic and hydrodynamic properties of the QGP are described by Quantum Chromodynamics (QCD) and can be studied in heavy-ion collisions. Despite being a key thermodynamic parameter, the QGP temperature is still poorly known. Thermal lepton pairs (e+e− and μ+μ−) are ideal penetrating probes of the true temperature of the emitting source, since their invariant-mass spectra suffer neither from strong final-state interactions nor from blue-shift effects due to rapid expansion. Here we measure the QGP temperature using thermal e+e− production at the Relativistic Heavy Ion Collider (RHIC). The average temperature from the low-mass region (in-medium ρ0 vector-meson dominant) is (1.99±0.24)×1012 K, consistent with the chemical freeze-out temperature from statistical models and the phase transition temperature from LQCD. The average temperature from the intermediate mass region (above the ρ0 mass, QGP dominant) is significantly higher at (3.40±0.55)×1012 K. This work provides essential experimental thermodynamic measurements to map out the QCD phase diagram and understand the properties of matter under extreme conditions.
With the STAR experiment at RHIC, we characterize √sNN = 200 GeV p+Au collisions by event activity (EA) measured within the pseudorapidity range η∈[−5,−3.4] in the Au-going direction and report correlations between this EA and hard- and soft-scale particle production at mid-rapidity (η∈[−1,1]). At the soft scale, charged particle production in low-EA p+Au collisions is comparable to that in \pp collisions and increases monotonically with increasing EA. At the hard scale, we report measurements of high transverse momentum (pT) jets in events of different EAs. In contrast to the soft particle production, high-pT particle production and EA are found to be inversely related. To investigate whether this is a signal of jet quenching in high-EA events, we also report ratios of pT imbalance and azimuthal separation of dijets in high- and low-EA events. Within our measurement precision, no significant differences are observed, disfavoring the presence of jet quenching in the highest 30% EA p+Au collisions at √sNN = 200 GeV.
Atomic nuclei are self-organized, many-body quantum systems bound by strong nuclear forces within femtometer-scale space. These complex systems manifest a diverse set of shapes~, traditionally explored via non-invasive spectroscopic techniques at low energies. Their instantaneous shapes, obscured by long-timescale quantum fluctuations, are considered not directly observable at low energy. We introduce a complementary method, collective flow assisted nuclear shape imaging, to image the nuclear global shape by colliding them at ultrarelativistic speeds and analyzing the collective response of outgoing debris. This technique captures a collision-specific snapshot of the spatial matter distribution in the nuclei, which, through the hydrodynamic expansion, leaves imprints on the particle momentum distribution patterns observed in detectors. We benchmark this method in collisions of ground state Uranium-238 nuclei, known for its elongated, axial-symmetric shape. Our findings, while confirming an overall deformation broadly consistent with prior low-energy experiments, also indicate a small deviation from axial symmetry in the nuclear ground state. This approach marks a new way of imaging nuclei, especially those with uncertain shape characteristics, and refines initial conditions in high-energy nuclear collisions. It tackles the important issue of nuclear structure evolution across various energy scales.
Experimental data from the NA49 collaboration show an unexpectedly steep rise of the rapidity width of the ϕ meson as function of beam energy, which was suggested as possible interesting signal for novel physics. In this work we show that the Ultra-relativistic Quantum-Molecular-Dynamics (UrQMD) model is able to reproduce the shapes of the rapidity distributions of most measured hadrons and predicts a common linear increase of the width for all hadrons. Only when following the exact same analysis technique and experimental acceptance of the NA49 and NA61/SHINE collaborations, we find that the extracted value of the rapidity width of the ϕ increases drastically for the highest beam energy. We conclude that the observed steep increase of the ϕ rapidity width is a problem of limited detector acceptance and the simplified Gaussian fit approximation.
Inflammation is a regulated reaction of the body to control a threat such as infection or injury. An efficient resolution of inflammation is critical to prevent the development of chronic inflammation and to restore tissue homeostasis. Macrophages (Mf) play a crucial role in the onset, but also in the resolution of inflammation, because they phagocytose and eliminate pathogens and tissue debris. Efficient efferocytosis, i.e. the engulfment of apoptotic cells, represents an important trigger for the onset of the resolution response and contributes to the pro-resolving reprogramming of Mf. Despite the importance of post- transcriptional modes of regulation during the resolution phase and translational control as a key node modulating gene expression in immune cells, relevant translational alterations remain largely elusive.
In the present study, I aimed to identify translationally regulated targets in inflammatory primary murine Mf upon resolution-promoting efferocytosis. To this end, I used total RNA-sequencing as well as de novo proteomics analyses to determine global transcriptional and translational changes. Sequencing data confirmed that efferocytosis induced a pro-resolution signature in inflammatory Mf and pointed towards translational regulation because the related integrated stress response was enriched upon efferocytosis. While changes of gene expression between efferocytic and non-efferocytic Mf appeared rather small at the transcriptional level, I observed considerable differences at the level of de novo synthesized proteins. This finding suggests a regulation at the level of translation. Furthermore, the tight connection between translational and metabolic changes was confirmed by enriched metabolism-associated terms of targets upregulated by efferocytosis at both RNA and de novo protein level. Interestingly, analysis of translationally regulated targets in response to inflammatory stimulation showed reduced translation for most targets, with only little impact of efferocytosis. Among those targets, I identified pro-resolving matrix metallopeptidase 12 (Mmp12) as a novel candidate, which showed translational repression during early inflammation and translational increase during the resolution phase. Noteworthy, a first indicator for a potential translation regulatory component of Mmp12 were the extremely high mRNA levels and not overly high de novo protein levels. Validation experiments recapitulated a slight elevation of Mmp12 mRNA expression and a significant downregulation of MMP12 intracellular protein levels in inflammatory Mf, as observed in the RNA-seq and de novo proteomics datasets. To investigate whether the discrepancy in mRNA and protein expression were due to changes in translation, I applied polysomal fractionation analysis to determine the translational status of Mmp12. Inflammatory Mf displayed a significantly lower relative Mmp12 mRNA abundance in the late polysomes compared to naïve Mf, suggesting reduced translational efficiency upon inflammatory stimulation. Consequently, extracellular MMP12 levels in the supernatant of inflammatory Mf decreased, although with a slight delay.
The functional impact of attenuated Mmp12 translation upon inflammatory stimulation was assessed in migration assays. While siRNA-mediated knockdown of Mmp12 did not alter Mf migration on uncoated plates, it increased migration 3-fold on matrigel/elastin-coated plates. Importantly, the increase in migrated distance driven by siMmp12 could be lowered by the addition of exogenous recombinant MMP12 protein. In line with reduced Mmp12 translation and MMP12 protein in inflammatory Mf, I observed a significant increase in cell migration on matrigel/elastin-coated plates, while it remained unaltered on uncoated plates. Consequently, Mf elastase MMP12 degrades elastin, thereby cell migration along elastin fibers is diminished. In inflammatory Mf, Mmp12 is translationally downregulated, thereby enhancing the migratory capacity.
In summary, the present study identifies a substantial contribution of translational regulation in the course of inflammation shown by high changes between inflammatory naïve and efferocytic Mf at the de novo proteomic level. Specifically, I was able to determine the translational regulation of pro-resolving Mmp12, which is repressed during early inflammation and recovers during the resolution phase. Functionally, translational control of MMP12 emerged as a strategy to alter the migratory properties of Mf, enabling enhanced, matrix- dependent migration of Mf during the early inflammatory phase, while restricting migration during the resolution phase.
In einer Welt voller Mehrdeutigkeiten führt ein Mangel an Ambiguitätstoleranz zu Misstrauen im Umgang mit der Vielzahl an Fakten, denen man sich täglich in vielerlei Gestalt gegenübersieht. Vielmehr verstärkt diese Informationsflut sogar bereits vorhandene Ängste vor der Zukunft. Gleichzeitig schwindet die Kompetenz im Umgang mit Texten, die versuchen, den prekären Zustand unserer Zeit erfahrbar zu machen. Der Beitrag entwirft ein Forschungsprogramm, das sich Lösungen für diese Probleme anzunähern versucht. Allen voran sind die kulturwissenschaftliche Forschung und im Zuge des 'cultural turn' eine Reihe von Forschungsrichtungen, die modernes Denken auf den Prüfstand stellen und sich gleichzeitig mit gesellschaftlichen Grundfragen und Herausforderungen unserer Zeit beschäftigen. Damit bergen sie praktische Lösungen in sich, die durch ästhetische Szenarien vermittelt werden können. Es wird der Frage nachgegangen, welche Rolle Literatur und Literaturwissenschaft(ler:innen) in diesen Prozessen einnehmen (müssen), sowie nach Möglichkeiten für Leser:innen und Autor:innen, Narrative mitzugestalten und so in der Gegenwart an der Zukunft produktiv mitzuarbeiten.
The human immune system is determined by the functionality of the human lymph node. With the use of high-throughput techniques in clinical diagnostics, a large number of data is currently collected. The new data on the spatiotemporal organization of cells offers new possibilities to build a mathematical model of the human lymph node - a virtual lymph node. The virtual lymph node can be applied to simulate drug responses and may be used in clinical diagnosis. Here, we review mathematical models of the human lymph node from the viewpoint of cellular processes. Starting with classical methods, such as systems of differential equations, we discuss the values of different levels of abstraction and methods in the range from artificial intelligence techniques formalism.
Using data samples with an integrated luminosity of 4.67 fb−1 collected by the BESIII detector operating at the BEPCII collider, we search for the process e+e−→η′ψ(2S) at center-of-mass energies from 4.66 to 4.95 GeV. No significant signal is observed, and upper limits for the Born cross sections σB(e+e−→η′ψ(2S)) at the 90\% confidence level are determined.
We report the first amplitude analysis of the decays D0→π+π−η and D+→π+π0η using a data sample taken with the BESIII detector at the center-of-mass energy of 3.773 GeV, corresponding to an integrated luminosity of 7.9 fb−1. The contribution from the process D0(+)→a0(980)+π−(0) is significantly larger than the D0(+)→a0(980)−(0)π+ contribution. The ratios B(D0→a0(980)+π−)/B(D0→a0(980)−π+) and B(D+→a0(980)+π0)/B(D+→a0(980)0π+) are measured to be 7.5+2.5−0.8stat.±1.7syst. and 2.6±0.6stat.±0.3syst., respectively. The measured D0 ratio disagrees with the theoretical predictions by orders of magnitudes, thus implying a substantial contribution from final-state interactions.
We investigate the early time dynamics of heavy ion collisions studying the time evolution of the energy-momentum tensor as well as energy-momentum correlations within a uniformly thermalizing holographic QGP. From these quantities, we suggest a far-from equilibrium definition of shear viscosity, which is a crucial property of QCD matter as it significantly determines the generation of elliptic flow already at early times. During an exemplary initial heating phase of the holographic QGP the shear viscosity of entropy density ratio decreases down to 60%, followed by an overshoot to 110% of the near-equilibrium value, η/s=1/(4π). Implications for the QCD QGP are discussed. Subsequently, we consider a holographic QGP which is Bjorken-expanding. Its energy-momentum tensor components have a known hydrodynamic attractor to which all time evolutions collapse independent of the initial conditions. Based on this, we propose a definition for a far from equilibrium speed of sound, and analytically compute its hydrodynamic attractor. Subjecting this Bjorken-expanding plasma to an external magnetic field and an axial chemical potential, we study the chiral magnetic effect far from equilibrium.
Animal agriculture is responsible for at least 16.5% of global yearly CO2e (carbon dioxide equivalent) emissions (Twine 2021: 3) and thus partially causal for the corresponding climate change, and its disastrous consequences for millions (Romanello et al. 2023: 1-2). At the same time, animal agriculture restricts and damages the bodily autonomy of animals regularly (Hampton et al. 2021: 28) which could be unethical depending on the underlying ethical theory. The policy option of veganism by law is, nevertheless, rarely considered. The definitions of veganism range from an individual ethic of the abstention from consuming animal products to a political philosophy calling for the abolition of animal agriculture (Mancilla 2016: 1-3). Because veganism through the cessation of animal agriculture could be the policy solution to the aforementioned issues concerning the rights of present and future generations affected by climate change and the rights of animals, I explore arguments for and against the implementation of veganism by law.
Although a veganized agriculture would provide 52% of the required emission reductions for the 2°C target of the Paris climate accord (Eisen and Brown 2022: 6), and could allow for greater animal welfare, current policies of many governments promote the opposite. For example, 82% of the subsidies of the European Union’s Common Agricultural Policy are routed towards the production of animal products and animal feed (Kortleve et al. 2024: 1-2). Moreover, for American adults the U.S. Department of Agriculture and the U.S. Department of Health and Human Services (2020: 96) promotes the consumption of 720ml of cow milk or other dairy per day and recommends a protein intake through meat and eggs between 652 g and 936 g per week.
In this bachelor thesis I outline the current state of animal agriculture, its emissions and the associated harm towards animals and humans. The empirical findings are dissected ethically with a consequentialist approach and a deontological approach. The ethical analysis concerning the decisions of individuals is then converted into a political philosophy regarding the duties of states towards present and future generations and animals including corresponding policy implications.
The normative argument is mainly based on the example of industrialized animal agricul-ture, the area where most of the interaction between animals and humans occurs. Nevertheless, other sectors where animals are used for human consumption or entertainment are discussed in less detail, in order to analyze the arguments for veganism by law.
In short, using the recommended political argument structure of Abel et al. (2021: 6) the following hypothesis acts as the basis for the political and philosophical discussion and is revised where necessary:
Moral claims: The state should protect present and future generations and animal rights.
Empirical claims: Animal agriculture is a major contributor to climate change and its corresponding effects and violates the wellbeing of animals regularly.
Conclusion: The state should enforce veganism by law.
Kriminalität und deren Bekämpfung unterliegen seit jeher permanenten Veränderungen. Durch den gesellschaftlichen Wandel und die damit einhergehende Weiterentwicklung von technischen Möglichkeiten werden fortlaufend neue Modi Operandi ermöglicht, auf welche die Polizei mit teilweise ebenfalls neuen kriminalistischen Mitteln reagiert. Entwicklungen in der Kriminalität und der Kriminalistik können somit als sich gegenseitig beeinflussende Prozesse angesehen werden. Allerdings findet dieser Wandel nicht kontinuierlich statt, sondern tritt in Schüben auf, die in der Regel auf besonders kriminalitätsrelevante Ereignisse zurückzuführen sind. In diesem Zusammenhang verändert sich die Polizei, sowohl in ihrer Organisation als auch in ihren Analysen zur Entwicklung der Kriminalität und in ihren Methoden zu deren Bekämpfung, teilweise grundlegend.
In einem ersten, konzeptionellen Teil wird anhand von ausgewählten Kriminalitätsformen und den damit verbundenen Ermittlungspraktiken ein Überblick der gegenseitigen Beeinflussung von Kriminalität und Kriminalistik gegeben. Darauf aufbauend wird im zweiten Teil der aktuelle Stand dieses 'Evolutionsprozesses' anhand der Implementierung von Predictive-Policing-Systemen in die Polizeiarbeit diskutiert. Denn darin zeigt sich nahezu idealtypisch die gegenseitige Beeinflussung von Kriminalität und Kriminalistik wie auch der digitale Wandel und die damit verbundenen Gefahren. Polizeiarbeit scheint sich im Ergebnis dieser Veränderungsprozesse von einer ursprünglich eher auf den Einzelfall bezogenen, subjektiven Sichtweise auf Kriminalität und deren Verfolgung (zum Beispiel durch kriminalistische Taktik und List in der Vernehmung) auf eine nunmehr eher allgemeine, abstraktere Betrachtung zu verschieben ('Abstract Police').
Polygene Risikoscores (PRS) integrieren zahlreiche Einzelnukleotid-Polymorphismen (SNP) von meist geringer Effektstärke, um Auskunft über das Erkrankungsrisiko bestimmter Krankheiten zu geben. In dieser Arbeit wurde der PRS zur genetisch generalisierten Epilepsie (GGE) von Leu et al. aus dem Jahr 2019 untersucht, um festzustellen, ob über das Erkrankungsrisiko hinaus noch Korrelationen mit weiteren phänotypischen Eigenschaften von Patienten bestehen. Der Nachweis solcher Zusammenhänge würde eine Prädiktionsfähigkeit des GGE-PRS demonstrieren, die perspektivisch ein Potential für dessen klinische Anwendbarkeit, beispielsweise im Sinne der personalisierten Medizin, aufzeigen könnte.
Die Identifizierung neuer Korrelationen sollte durch Vergleich der Phänotypen von zwei Gruppen von GGE-Patienten mit extrem hohen, beziehungsweise extrem niedrigen PRS-Werten erfolgen. Hierfür wurden von 2256 Patienten aus der Datenbank von Epi25, einem internationalen Forschungskollaborativ zur Erforschung der Relevanz genetischer Faktoren bei der Entwicklung von Epilepsie, die Patienten mit den höchsten (n=59) und den niedrigsten (n=49) GGE-PRS-Werten ausgewählt. Für diese 108 Patienten wurden retrospektive klinische Daten von den jeweiligen Behandlungszentren akquiriert. Hierzu wurde den Studienleitern der Zentren ein Questionnaire mit Fragen zu zahlreichen phänotypischen Parametern der Patienten übermittelt. Die Rücklaufrate war mit 54% gut.
Die so eingeholten Patientendaten wurden anschließend mittels Exaktem Test nach Fisher und Wilcoxon-Rangsummentest statistisch analysiert, um Unterschiede zwischen den Phänotypen beider Gruppen nachzuweisen. Im Falle der Pharmakoresistenz zeichneten sich hierbei zunächst signifikante Unterschiede ab, die ein selteneres Auftreten dieser Eigenschaft für Patienten mit hohen GGE-PRS-Werten implizierten. Diese Ergebnisse waren jedoch nach einer Bonferroni-Korrektur und bei Validierung in einer größeren Kohorte (n=825) nicht mehr signifikant. Für die anderen untersuchten Parameter waren ebenfalls keine signifikanten Unterschiede nachweisbar.
Das Ergebnis, dass für keinen der untersuchten Parameter signifikante Differenzen bestanden, obwohl zwei Kohorten mit extrem gegensätzlichen PRS-Werten untersucht wurden, spricht gegen eine Verwendung des aktuell verfügbaren GGE-PRS als prädiktiver Biomarker über das Erkrankungsrisiko hinaus und somit gegen dessen klinische Anwendbarkeit. Jedoch können die nicht-signifikanten Korrelationen im Falle der Pharmakoresistenz als Hinweis verstanden werden, dass im Bereich der Pharmakotherapie Zusammenhänge zwischen Score und Phänotyp bestehen könnten, die weiterer Untersuchungen in zukünftigen Studien bedürfen. Bei Verwendung eines verbesserten GGE-PRS mit zusätzlichen risikoassoziierten SNP und verfeinerter Wichtung der Effektstärken sowie größerer Kohorten könnten in diesem Bereich möglicherweise auch signifikante Zusammenhänge nachweisbar werden.
Die Appendizitis stellt mit einer Inzidenz von 115 pro 100000 Einwohnern in Deutschland eine der häufigsten Ursachen für ein akutes Abdomen dar. Bakterielle Infektionen sind ein wesentlicher Faktor für die postoperative Morbidität nach Appendektomie.
Ziel dieser prospektiven Studie war es, das Keimspektrum und insbesondere die Prävalenz resistenter Keime bei der akuten Appendizitis zu bestimmen und die Auswirkungen resistenter Keime auf das Auftreten infektiöser Komplikationen zu analysieren. Alle erwachsenen Patient*innen mit akuter Appendizitis, die zwischen April 2022 und Juli 2023 am Universitätsklinikum Frankfurt operativ behandelt wurden, wurden prospektiv eingeschlossen. Das Keimspektrum der Appendix und die Häufigkeit von MRE in Rektalabstrichen wurden analysiert. Die klinischen Daten wurden extrahiert, um die Korrelation des Keimspektrums mit dem Auftreten von postoperativen Komplikationen, Dauer und Art der Antibiotikatherapie und dem postoperativen Verlauf zu evaluieren. 30 Tage nach der Operation wurde ein Follow-up durchgeführt. Insgesamt wurden 105 Patient*innen in die Studie eingeschlossen.
In den Appendixabstrichen gelang ein Erregernachweis bei 67,6 % aller Fälle. Hierbei betrug die Prävalenz von Keimen mit Antibiotikaresistenz 43,8 %, Multipler-Antibiotikaresistenz (MAR) 27,6 % und bei 5,7 % der Fälle gelang ein MRE-Nachweis nach CDC im Appendixabstrich. Beim MRE-Screening konnten im Rektalabstrich bei 11,4 % der Patient*innen ein MRE nachgewiesen werden. In vier Fällen zeigte sich eine Übereinstimmung zwischen rektalen und appendikulären Abstrich, somit betrug die Sensitivität des MRE-Screenings für einen Nachweis multiresistenter Keime in der Appendix lediglich 33,3 % bei einer Spezifität von 86,7 %. In einer univariaten Analyse konnte das Vorliegen eines Diabetes mellitus als Risikofaktor für das Auftreten von Keimen mit Cefuroxim- Resistenz, Multipler-Antibiotikaresistenz (MAR) und MRE identifiziert werden.
Insgesamt erhielten 47,6 % aller Patient*innen postoperativ eine Antibiotikatherapie, von denen erfolgte bei 46 % eine Umstellung der empirischen Antibiose auf eine antibiogramm-gerechte Therapie nach Erhalt des mikrobiologischen Befundes. Insbesondere Patient*innen mit komplizierter Appendizitis erhielten postoperativ eine Antibiotikatherapie, wobei 28 % eine Antibiotikaeskalation benötigten.
Patient*innen, deren Appendixabstriche eine Resistenz gegen maximal ein Antibiotikum aufwiesen, wurden als nicht multiple Resistenz (Nicht-MAR) definiert und mit Patient*innen verglichen, deren Keime mit multipler Antibiotikaresistenz (MAR) waren. Die MAR-Gruppe zeigte im Vergleich zur Nicht-MAR-Gruppe eine höhere Inzidenz postoperativer Komplikationen, insbesondere eine erhöhte Inzidenz von Clavien-Dindo Grad 3 Komplikationen sowie von tiefen postoperativen Wundinfektionen (CDC Grad A3). Weiterhin benötigten Patient*innen der MAR-Gruppe häufiger eine postoperative Antibiotikatherapie und es erfolgte häufiger eine Eskalation der antibiotischen Therapie. Dies ging auch mit einem signifikant verlängerten Krankenhausaufenthalt einher.
Zusammenfassend kann festgestellt werden, dass eine bakterielle Infektion mit Multipler Antibiotikaresistenz bei der akuten Appendizitis häufig auftritt und die Morbidität nach Appendektomie beeinflusst. Die mikrobiologische Untersuchung mittels Appendixabstrich bei operativ behandelten Fällen von akuter komplizierter Appendizitis, die eine postoperative Antibiotikatherapie erfordern, könnte somit eine gezielte und kürzere Antibiotikatherapie ermöglichen. Dies könnte dazu beitragen, längere Krankenhausaufenthalte zu vermeiden, den unnötigen Einsatz unwirksamer Antibiotika zu reduzieren und die Kosten im Gesundheitswesen zu senken.
