USP22 controls necroptosis by regulating receptor-interacting protein kinase 3 ubiquitination

Dynamic control of ubiquitination by deubiquitinating enzymes is essential for almost all biological processes. Ubiquitin-specific peptidase 22 (USP22) is part of the SAGA complex and catalyzes the removal of mono-ubiquitination from histones H2A and H2B, thereby regulating gene transcription. However, novel roles for USP22 have emerged recently, such as tumor development and cell death. Apart from apoptosis, the relevance of USP22 in other programmed cell death pathways still remains unclear. Here, we describe a novel role for USP22 in controlling necroptotic cell death in human tumor cell lines. Loss of USP22 expression significantly delays TNFα/Smac mimetic/zVAD.fmk (TBZ)-induced necroptosis, without affecting TNFα-mediated NF-κB activation or extrinsic apoptosis. Ubiquitin remnant profiling identified receptor-interacting protein kinase 3 (RIPK3) lysines 42, 351, and 518 as novel, USP22-regulated ubiquitination sites during necroptosis. Importantly, mutation of RIPK3 K518 reduced necroptosis-associated RIPK3 ubiquitination and amplified necrosome formation and necroptotic cell death. In conclusion, we identify a novel role of USP22 in necroptosis and further elucidate the relevance of RIPK3 ubiquitination as crucial regulator of necroptotic cell death.
Oncogenic transformation of lung epithelial cells is a multistep process, frequently starting with the inactivation of tumour suppressors and subsequent development of activating mutations in proto-oncogenes, such as members of the PI3K or MAPK families. Cells undergoing transformation have to adjust to changes, including altered metabolic requirements. This is achieved, in part, by modulating the protein abundance of transcription factors. Here, we report that the ubiquitin carboxyl-terminal hydrolase 28 (USP28) enables oncogenic reprogramming by regulating the protein abundance of proto-oncogenes such as c-JUN, c-MYC, NOTCH and ∆NP63 at early stages of malignant transformation. USP28 levels are increased in cancer compared with in normal cells due to a feed-forward loop, driven by increased amounts of oncogenic transcription factors such as c-MYC and c-JUN. Irrespective of oncogenic driver, interference with USP28 abundance or activity suppresses growth and survival of transformed lung cells. Furthermore, inhibition of USP28 via a small-molecule inhibitor resets the proteome of transformed cells towards a ‘premalignant’ state, and its inhibition synergizes with clinically established compounds used to target EGFRL858R-, BRAFV600E- or PI3KH1047R-driven tumour cells. Targeting USP28 protein abundance at an early stage via inhibition of its activity is therefore a feasible strategy for the treatment of early-stage lung tumours, and the observed synergism with current standard-of-care inhibitors holds the potential for improved targeting of established tumours.

Summary
The endosomal-lysosomal system is a series of organelles in the endocytic pathway that executes trafficking and degradation of proteins and lipids and mediates the internalization of nutrients and growth factors to ensure cell survival, growth, and differentiation. Here, we reveal regulatory, non-proteolytic ubiquitin signals in this complex system that are controlled by the enigmatic deubiquitinase USP32. Knockout (KO) of USP32 in primary hTERT-RPE1 cells results among others in hyperubiquitination of the Ragulator complex subunit LAMTOR1. Accumulation of LAMTOR1 ubiquitination impairs its interaction with the vacuolar H+-ATPase, reduces Ragulator function, and ultimately limits mTORC1 recruitment. Consistently, in USP32 KO cells, less mTOR kinase localizes to lysosomes, mTORC1 activity is decreased, and autophagy is induced. Furthermore, we demonstrate that depletion of USP32 homolog CYK-3 in Caenorhabditis elegans results in mTOR inhibition and autophagy induction. In summary, we identify a control mechanism of the mTORC1 activation cascade at lysosomes via USP32-regulated LAMTOR1 ubiquitination.
The mitotic spindle assembly checkpoint (SAC) maintains genome stability and marks an important target for antineoplastic therapies. However, it has remained unclear how cells execute cell fate decisions under conditions of SAC‐induced mitotic arrest. Here, we identify USP9X as the mitotic deubiquitinase of the X‐linked inhibitor of apoptosis protein (XIAP) and demonstrate that deubiquitylation and stabilization of XIAP by USP9X lead to increased resistance toward mitotic spindle poisons. We find that primary human aggressive B‐cell lymphoma samples exhibit high USP9X expression that correlate with XIAP overexpression. We show that high USP9X/XIAP expression is associated with shorter event‐free survival in patients treated with spindle poison‐containing chemotherapy. Accordingly, aggressive B‐cell lymphoma lines with USP9X and associated XIAP overexpression exhibit increased chemoresistance, reversed by specific inhibition of either USP9X or XIAP. Moreover, knockdown of USP9X or XIAP significantly delays lymphoma development and increases sensitivity to spindle poisons in a murine Eμ‐Myc lymphoma model. Together, we specify the USP9X–XIAP axis as a regulator of the mitotic cell fate decision and propose that USP9X and XIAP are potential prognostic biomarkers and therapeutic targets in aggressive B‐cell lymphoma.
Aim: To study the changes in leiomyoma volume following uterine artery embolization (UAE) and to correlate these changes with the initial leiomyoma volume and location within the uterus and to evaluate the impact of preprocedural prediction of the best tube angle obliquity for visualization of the uterine artery origin using 3D-reconstructed contrast-enhanced MR angiography (CE-MRA) on the radiation dose, fluoroscopy time and contrast medium volume used during UAE. Materials and Methods: The study was performed in two parts. The first part was retrospectively done on 28 patients (age range: 37-57 years, mean: 48 years, SD: 4.81) in whom UAE was performed. All leiomyomas in all patients were evaluated. In total, 84 leiomyomas were evaluated. MRI studies were performed before, 3 months and 1 year after UAE. The volumes and location of each leiomyoma in each patient were evaluated in consensus by two radiologists. The second part included 40 consecutive patients (age range: 37-56 years, mean: 46 years, SD: 4.49) and was done in a controlled prospective/retrospective manner. In 20 sample patients (prospective part) pre-procedural prediction of the best tube angle obliquity was predicted using 3D-reconstructed CE-MRA and provided to the interventionalist. 3D-reconstruction was done using Inspace application. The radiation dose, fluoroscopy time and contrast medium volume for those patients were compared with the data of the last 20 procedures (control) performed by the same interventionalist (retrospective part). Results: For the first part the mean pre-embolization volume was 51.6 cm3 range:0.72-371.1cm3, SD=79.3). At 3-month follow-up 83 (98.8%) leiomyomas showed a mean volume reduction of 52.62% (range: 12.79–96.67%, SD=21.85) and 1 leiomyoma (1.2%) increased in volume. At 1-year follow-up 5 (6%) leiomyomas were not detectable, 72 (85.7%) showed a further mean of 20.5% (range: 2.52–58.72%, SD=11.92) volume reduction compared to the 3-month follow-up volume and 7 (8.3%) leiomyomas increased in volume. A statistically significant (p=0.026 at 3-month, p=0.0046 at 1-year) difference in percentage of volume change was observed based on leiomyoma location; submucous leiomyomas showed the largest volume reduction. The initial leiomyoma volume showed a weak negative correlation (Spearman's correlation-coefficient =-0.35 at 3m and -0.36 at 1y) with the leiomyoma volume change. For the second part the tube angle prediction resulted in a significant reduction of the radiation dose utilized (p<0.001), fluoroscopy time (p=0.002) and contrast medium volume (p<0.001) for the sample patients when compared with the control patients. The overall radiation dose was reduced from a mean of 11044 μGym2 to a mean of 4172.5 μGym2, fluoroscopy time was reduced from a mean of 15.45 minutes to 8.81 minutes and contrast medium volume was reduced from a mean of 135 ml to 75 ml. Conclusion: UAE results in significant leiomyoma volume reduction at 3-month and 1- year follow-up. The leiomyoma location plays an important role in volume changes while the initial leiomyoma volume plays a minor role. Pre-procedural prediction of the best tube angle obliquity for visualization of the origin of the uterine artery using 3D-reconstructed CE-MRA results in a significant reduction of the radiation dose, fluoroscopy time and contrast medium volume used during UAE.
The plaque reduction neutralization test (PRNT) is a preferred method for the detection of functional, SARS-CoV-2 specific neutralizing antibodies from serum samples. Alternatively, surrogate enzyme-linked immunosorbent assays (ELISAs) using ACE2 as the target structure for the detection of neutralization-competent antibodies have been developed. They are capable of high throughput, have a short turnaround time, and can be performed under standard laboratory safety conditions. However, there are very limited data on their clinical performance and how they compare to the PRNT. We evaluated three surrogate immunoassays (GenScript SARS-CoV-2 Surrogate Virus Neutralization Test Kit (GenScript Biotech, Piscataway Township, NJ, USA), the TECO® SARS-CoV-2 Neutralization Antibody Assay (TECOmedical AG, Sissach, Switzerland), and the Leinco COVID-19 ImmunoRank™ Neutralization MICRO-ELISA (Leinco Technologies, Fenton, MO, USA)) and one automated quantitative SARS-CoV-2 Spike protein-based IgG antibody assay (Abbott GmbH, Wiesbaden, Germany) by testing 78 clinical samples, including several follow-up samples of six BNT162b2 (BioNTech/Pfizer, Mainz, Germany/New York, NY, USA) vaccinated individuals. Using the PRNT as a reference method, the overall sensitivity of the examined assays ranged from 93.8 to 100% and specificity ranged from 73.9 to 91.3%. Weighted kappa demonstrated a substantial to almost perfect agreement. The findings of our study allow these assays to be considered when a PRNT is not available. However, the latter still should be the preferred choice. For optimal clinical performance, the cut-off value of the TECO assay should be individually adapted.
Utility of the new cobas HCV test for viral load monitoring during direct-acting antiviral therapy

Background: The COBAS AmpliPrep/COBAS TaqMan assay HCV (CAP/CTM) is widely used in clinical routine for HCV testing. Recently, the new cobas HCV test was established for high throughput testing with minimal operator intervention. As different assays may yield different quantitative/qualitative results that possibly impact treatment decisions, the aim of this study was to externally evaluate the cobas HCV test performance in comparison to CAP/CTM in a clinically relevant setting.
Methods: Serum samples were obtained from 270 patients who received direct acting antiviral therapy with different treatment regimens at two study sites (Hannover and Frankfurt) in 2016. Overall, 1545 samples (baseline, on-treatment and follow-up) were tested in parallel by both assays.
Results: The mean difference between cobas HCV and CAP/CTM for the quantification of HCV RNA was 0.008 log10 IU/ml HCV RNA (95% limits of agreement: -0.02–0.036) showing excellent agreement of both assays. With respect to clinical cut offs (HCV RNA detectable vs. target not detected and HCV RNA above the lower limit of quantification (LLOQ) vs. <LLOQ), discordant results were obtained in 9.5% and 4.6%, respectively; the greatest differences were observed during early stages of antiviral therapy (week 1, week 2 and week 4), but none were statistically significant. Overall percent agreement for SVR between cobas HCV and CAP/CTM at the 15 IU/ml cutoff was 99.2% (95%CI 92.7%-100%).
Conclusion: The performance of the new cobas HCV test was comparable to CAP/CTM in a clinical setting representing a large patient population with HCV GT 1 and 3 treated with DAAs.
Objectives: We aimed to investigate the contemporary usage rate and habits of the WHO Surgical Safety Checklist (SSC) in German urological departments.
Methods: We designed a 26-item questionnaire that was sent to all urological departments in Germany. The primary aim of this study was to evaluate the usage rate of the SSC. Secondary aims were to compare perioperative characteristics of users vs. non-users of the SSC and to assess circumstances of the SSC application.
Results: A total of 213 of 234 (91 %) urological departments were users of the SSC, and 21 (9 %) were non-users. SSC users had more often a standard protocol, took less time and had fewer people involved for checking perioperative patient data compared to non-users. Financial budgeting for the SSC existed in 55 (24 %) departments and for patient safety in 73 (32 %) departments.
Conclusions: The usage rate of the SSC in urological departments in Germany is high despite restricted financial budgeting. Users of the SSC profit by saving time and manpower for checking perioperative patient data.
Human endogenous retrovirus (HERV) genomes are chromosomally integrated in all cells of an individual. They are normally transcriptionally silenced and transmitted only vertically. Enhanced expression of HERV-K accompanied by the emergence of anti-HERV-K-directed immune responses has been observed in tumor patients and HIV-infected individuals. As HERV-K is usually not expressed and immunological tolerance development is unlikely, it is an appropriate target for the development of immunotherapies. We generated a recombinant vaccinia virus (MVA-HKenv) expressing the HERV-K envelope glycoprotein (ENV), based on the modified vaccinia virus Ankara (MVA), and established an animal model to test its vaccination efficacy. Murine renal carcinoma cells (Renca) were genetically altered to express E. coli beta-galactosidase (RLZ cells) or the HERV-K ENV gene (RLZ-HKenv cells). Intravenous injection of RLZ-HKenv cells into syngenic BALB/c mice led to the formation of pulmonary metastases, which were detectable by X-gal staining. A single vaccination of tumor-bearing mice with MVA-HKenv drastically reduced the number of pulmonary RLZ-HKenv tumor nodules compared to vaccination with wild-type MVA. Prophylactic vaccination of mice with MVA-HKenv precluded the formation of RLZ-HKenv tumor nodules, whereas wild-type MVA-vaccinated animals succumbed to metastasis. Protection from tumor formation correlated with enhanced HERV-K ENV-specific killing activity of splenocytes. These data demonstrate for the first time that HERV-K ENV is a useful target for vaccine development and might offer new treatment opportunities for diverse types of cancer.
Nosocomial infectious diseases (e.g. influenza, pertussis) are a threat particularly for immunocompromised and vulnerable patients. Although vaccination of healthcare workers (HCWs) constitutes the most convenient and effective means to prevent nosocomial transmissions, vaccine uptake among HCWs remains unacceptably low. Worldwide, numerous studies have demonstrated that nurses have lower vaccination rates than physicians and that there is a relationship between receipt of vaccination by HCWs and knowledge. Measures to improve vaccination rates need to be profession-sensitive as well as specific in their approach in order to achieve sustained success.
Einführung: Seit 20 Jahren ist die Vagusnervstimulation (VNS) eine europaweit zugelassene invasive Therapieoption für therapieresistente Depressionen (TRD). Im Gegensatz zu geläufigeren Behandlungen wie EKT sind Kenntnisse über VNS sowohl in der Allgemeinbevölkerung als auch in Fachkreisen gering.
Methoden: In diesem narrativen Review geben wir eine klinisch und wissenschaftlich fundierte Übersicht über die VNS. Hypothesen zum Wirkmechanismus sowie die aktuelle Evidenzlage zur Wirksamkeit werden dargestellt. Das perioperative Management, das Nebenwirkungsprofil und die Nachbetreuung einschließlich Dosistitration werden beschrieben. Ein Vergleich über internationale Leitlinienempfehlungen zur VNS findet sich ebenfalls. Ferner formulieren wir Kriterien, die bei der Auswahl geeigneter Patienten hilfreich sind.
Ergebnisse: Die elektrischen Impulse werden über den N. vagus afferent weitergeleitet und stimulieren über verschiedene Wege ein neuromodulatorisches zerebrales Netzwerk. Viele Studien und Fallserien zeigten die Wirksamkeit von VNS als adjuvantes Verfahren bei TRD. Der Effekt tritt mit einer Latenz von 3 bis 12 Monaten ein und steigt möglicherweise mit der Dauer der VNS. Unter der Beachtung der Stimulationsempfehlungen sind die Nebenwirkungen für die meisten Patienten tolerabel.
Fazit: Die VNS ist eine zugelassene, wirksame und gut verträgliche Langzeittherapie für chronische und therapieresistente Depressionen. Weitere Sham-kontrollierte Studien über einen längeren Beobachtungszeitraum sind zur Verbesserung der Evidenz wünschenswert.
Background: There are no validated standardised clinical procedures for severity measurement of acute bronchitis in children. The "BSS-ped", a short version of the physician-rated assessment scale BSS (Bronchitis Severity Scale), can fill this gap, if it is valid.
Objective: To examine the scale´s validity.
Methods: Investigations were planned according to classical clinical-psychometric validity criteria including a formal competence evaluation of the scale´s authors and statistical analyses of data from 78 patients aged 1-6 and diagnosed with "acute bronchitis". Cross-validation was provided by analysis of data from 70 children with matching age, sex and diagnosis. All children were examined three times (day 0, 3-5 and 7) using the BSS-ped in addition to other clinical and psychometric monitoring procedures.
Results: The evidently high level of expertise of the scale’s authors substantiates pronounced content validity and relevance of the BSS-ped and its items. The validity criterion, i.e. to reflect the unidimensional severity of acute bronchitis and its change using the BSS-ped score, was fulfilled. There were substantial correlations with other scales measuring the current health-related quality of life, as well as satisfaction and success of treatment. Severity change prognoses for acute bronchitis under placebo and an active substance were correct. The BSS-ped was found to be a feasible instrument because it can be repeated at short intervals (minute range) without any special technical aids or extended training.
Conclusion: The BSS-ped is a valid procedure for measuring the severity of acute bronchitis in children.
Biological exploration of early biomarkers for chronic kidney disease (CKD) in (pre)diabetic individuals is crucial for personalized management of diabetes. Here, we evaluated two candidate biomarkers of incident CKD (sphingomyelin (SM) C18:1 and phosphatidylcholine diacyl (PC aa) C38:0) concerning kidney function in hyperglycemic participants of the Cooperative Health Research in the Region of Augsburg (KORA) cohort, and in two biofluids and six organs of leptin receptor-deficient (db/db) mice and wild type controls. Higher serum concentrations of SM C18:1 and PC aa C38:0 in hyperglycemic individuals were found to be associated with lower estimated glomerular filtration rate (eGFR) and higher odds of CKD. In db/db mice, both metabolites had a significantly lower concentration in urine and adipose tissue, but higher in the lungs. Additionally, db/db mice had significantly higher SM C18:1 levels in plasma and liver, and PC aa C38:0 in adrenal glands. This cross-sectional human study confirms that SM C18:1 and PC aa C38:0 associate with kidney dysfunction in pre(diabetic) individuals, and the animal study suggests a potential implication of liver, lungs, adrenal glands, and visceral fat in their systemic regulation. Our results support further validation of the two phospholipids as early biomarkers of renal disease in patients with (pre)diabetes.
Background: Transplantation of human corneal tissue is associated with the potential risk of transmittance of viral infections. In accordance with European directives and federal laws, in Germany each tissue donor has to be tested for infectious diseases such as hepatitis B and C virus (HBV and HCV) and human immunodeficiency virus (HIV) infection. However, most of the currently available CE-marked serologic and nucleic acid screening systems are only validated for antemortem blood. Methods: Twenty related and paired ante- and postmortem blood samples from cornea donors were obtained and subsequently analyzed for hepatitis B surface antigen (HBsAg), hepatitis B antibody (anti-HBc), anti-HCV, HCV RNA, anti-HIV-1/2, and HIV p24 Ag using Abbott test systems. The sera were also spiked with reference materials in concentrations giving low and high positivity for HBV, HCV, and HIV markers. Results: The spiked ante- and postmortem sera from related donors showed similar results for HBsAg, anti-HBc, anti-HCV, HCV RNA, anti-HIV, and HIV p24 Ag, indicating a high stability of viral markers in cadaveric specimens. Three cornea donors had a medical history of HBV infection and revealed anti-HBc at similar levels in the ante- and postmortem sera. In addition, there was a single postmortem sample demonstrating a weak signal of anti-HIV-1 and HIV-1 p24 Ag. False-positive or false-negative results were not detected. The results obtained with the Abbott ARCHITECT analyzer and Abbott RealTime HCV PCR showed no significant differences. Conclusion: The analyzed screening assays are suitable for the detection of infectious markers of HBV, HCV, and HIV at similar levels in spiked ante- and postmortem sera from cornea donors.
Das Chronic Care Model (CCM) stellt eine Zusammenfassung evidenzbasierter Erkenntnisse dar, um die Versorgung chronisch Erkrankter zu optimieren. Diverse Messinstrumente sind in den letzten Jahren zur Erfassung des Implementierungserfolges des CCM aus Sicht der Leistungserbringer und der Patienten entwickelt worden. Einen Ansatz zur Erfassung, inwiefern die im CCM beinhalteten Dimensionen angewendet werden, stellt das Patient Assessment of Chronic Illness Care (PACIC- 5A) dar. Dieses Instrument wurde im englischsprachigen Raum entwickelt und primär im amerikanischen Gesundheitssystem eingesetzt. Es besteht in der ursprünglichen Fassung aus 20 Items und erfragt in fünf Subskalen CCM-konkordante Aspekte: Patientenaktivierung, Leistungserbringerstruktur / Entscheidungshilfen, Zielsetzung, Problemlösung / Beratung, Follow-up / Koordination. Die erweiterte Version aus 26 Items erlaubt zusätzlich zur Beurteilung der Erfüllung des CCM die Analyse der Umsetzung im Sinne des 5A-Modells, eines behaviouristischen Ansatzes zur Verhaltensänderung. Ziel dieser Arbeit war es, den Fragebogen in die deutsche Sprache ohne Verlust des semantischen Inhaltes zu übersetzen und an einer primärmedizinischen Population im deutschen Gesundheitssystem zu evaluieren. Eine weitere Zielsetzung war, den Implementierungsgrad des CCM-Konstruktes im spezifischen Fall der Major Depression zu erkunden. Hierzu wurde das übersetzte PACIC-5A im Rahmen der im Institut für Allgemeinmedizin der Universitätsklinik Frankfurt durchgeführten ProMPT-Studie zur Betreuung von Patienten mit Major Depression an insgesamt 509 Patienten erprobt. Hiervon wurden 436 Patienten in die Studie eingeschlossen. Hierauf erfolgte die Überprüfung der psychometrischen Eigenschaften. Die statistischen Messergebnisse, hinsichtlich innerer Konsistenz (Cronbach´s α) und Trennschärfe (korrigierte Item-Skala-Korrelation), ergaben gute und mit der aktuellen Literatur übereinstimmende Resultate. Die konvergente Validität des PACIC-5A wurde anhand des primärmedizinisch validen Fragebogens EUROPEP (European Project on Patient Evaluation of General Practice Care) geprüft. Das Ergebnis zeigte hohe Korrelationen vor allem in den inhaltlich vergleichbaren Skalen, wie z.B. Skala „Patientenaktivierung“ aus dem PACIC-5A und Skala „Information und Unterstützung“ des EUROPEP. Die diskriminierenden Fähigkeiten des Fragebogens wurden durch die geringen korrelativen Eigenschaften mit Skalen des EUROPEP, welche andere Dimensionen erörtern, nachgewiesen. Zusätzlich ermöglichte der Vergleich der Mittelwerte der einzelnen Skalen des PACIC-5A zwischen dem Kontroll- und Interventionsarm eine zusätzliche Beurteilung der diskriminierenden Potenz. Das PACIC-5A ist bis jetzt vorrangig an Patienten mit chronischen Erkrankungen aus dem organischen Formenkreis (z.B. Osteoarthritis, Diabetes Mellitus) angewendet und validiert worden. Die kongruente Anwendung des PACIC-5A auf Erkrankte mit Major Depression ist ein limitierender Aspekt. Hier besteht weiterer Forschungsbedarf. Für eine optimale Patientenversorgung in der Primärmedizin ist es essentiell, primärmedizinisch valide Instrumente den Hausärzten zur Verfügung zu stellen.
Validierung einer neuen Messmethode zur direkten Bestimmung der Heparin-Konzentration im Blut

In dieser Arbeit wurde ein neues Verfahren zur Heparin-Bestimmung (LiSA-H, light scattering assay of heparin) evaluiert. Dieses wurde an der Universität Frankfurt a. M. am Institut für Biophysik entwickelt und ermittelt erstmals die direkte Heparin-Konzentration im Blutplasma. Durch die Analyse der Lichtstreuung einer Plasmaprobe wird die Bildung von Nanopartikeln aus Heparin und Protamin verfolgt. Die Lichtstreuintensität ist dabei proportional zu der in der Probe enthaltenen Heparin-Plasmakonzentration (Heparin-PK). Das Antikoagulans Heparin wird bei Herz-OPs mit Einsatz der Herz-Lungen-Maschine (HLM) verwendet und soll perioperativ eine lutgerinnselbildung in der HLM, sowie Thromboembolien im Patienten verhindern. Am OP-Ende wird die Wirkung durch Protamin antagonisiert, um eine suffiziente Gerinnung wieder herzustellen. Das derzeitige Gerinnungsmanagement basiert auf einem indirekten Messverfahren, der ACT (activated clotting time), welches starken Störeinflüssen, wie z.B. der Hypothermie, Hämodilution und bestimmten Medikamenten unterliegt. Durch die mögliche Falschdosierung der beiden Medikamente, steigt die Gefahr einer Blutung und Thrombose. LiSA-H soll in Zukunft eine zuverlässigere und kostengünstige „point of care― Analyse der Gerinnung und hierdurch eine gezielte Dosierung von Heparin und Protamin ermöglichen, die die Komplikationsrate verringert. In der vorliegenden Studie handelt es sich um eine offene, nicht kontrollierte, prospektive Multicenter-Studie, die mit 50 Patienten am Universitätsklinikum Frankfurt a.M. und 30 Patienten am Kinderherzzentrum Gießen durchgeführt wurde. Es wurde gezeigt, dass die durchschnittliche perioperative Heparin-PK bei Erwachsenen und Kindern bei ca. 5,4 I.E./ml liegt. Es wurde nachgewiesen, dass die Heparin-Clearance bei Kindern (~113 Min.) um das zweifache im Vergleich zu Erwachsenen (~254 Min.), erhöht ist. Besonders hervorzuheben ist die hohe Fehlerquote der ACT-Messung, die bei Erwachsenen in bis zu 1,8 % und bei Kindern in bis zu 20 % der Messungen keinen aussagekräftigen Wert lieferte. Das bedeutet, dass bei Kindern während einer OP bis zu zwei und bei Erwachsenen bis zu drei Stunden keine Information über den aktuellen Gerinnungszustand vorlag. Um eine Validierung der Messergebnisse vorzunehmen, wurden Rückstellproben mit dem Standardlaborverfahren PiCT (Prothrombinase induced clotting time) gemessen. Die Daten aus dem PiCT korrelieren mit den Ergebnissen aus der LiSA-H-Messung wesentlich besser (r² = 0,80), als mit der herkömmlichen ACT-Messmethode (r² = 0,57). Die ermittelten Heparin-PK und die ACT-Werte während einer OP wurden in Chronogrammen dargestellt. Es wurde gezeigt, dass in 30 % der OPs bei Erwachsenen und in 60 % bei Kindern die Messdaten aus der ACT und LiSA-H nur unzureichend synchron bei Nachdosierung mit Heparin anstiegen oder entsprechend der Heparin-Clearance im OP-Verlauf abfielen. Dies zeigte sich besonders kritisch während langandauernder, komplikationsreicher OPs, die einen erhöhten Blutverlust oder sogar Rethorakotomien nach sich zogen, in denen der ACT-Wert eine suffiziente Gerinnung nahe legte, die LiSA-HMessung aber eine noch hohe Heparin-PK nachwies. Erfahrungen aus den klinischen Studien zeigten, dass die Kombination aus der Messung der Heparin-PK und einer Gerinnungsanalyse bei einem ATIII-Mangel von Vorteil ist. Erst die Kombination aus einerseits mehrfach niedrig gemessener ACT-Werte, trotz ggf. Nachdosierungen von Heparin und andererseits ausreichend gemessener Heparin-PK im LiSA-H, kann einen ATIII-Mangelzustand aufdecken. Dadurch können Nach- bzw. Überdosierungen vermieden und damit die Wahrscheinlichkeit für postoperative Komplikationen verringert werden. Der wichtigste Einflussfaktor auf die LiSA-H-Messung ist die Hämodilution, die durch Einbeziehung des Patienten-Blutvolumens (z.B. mit der Nadler-Formel) durch mathematische Korrektur berücksichtigt werden kann. Patientenindividuelle Reaktionen auf gleiche Heparin- und Protamin-Dosierungen sowie eine patientenspezifische Heparin-Clearance zeigten in diesen Studien auf, dass das derzeitige Antikoagulationsmanagement mit den Dosierungsempfehlungen (Körpergewichtsbezogene Dosierung, 1:1 Dosierung von Protamin zur initialen Heparin-Dosis oder der summierten Heparin-Dosis, „pauschale― Nachdosierungen von 5.000 oder 10.000 I.E. Heparin bei ACT < 480) für eine optimale Dosierung der Medikamente unzureichend ist. In Outcome-Studien soll mit der LiSA-H-Messung Dosierungsempfehlungen von Heparin und Protamin ausgearbeitet werden. Außerhalb der Herz-Thorax-Chirurgie eröffnen sich weitere Möglichkeiten, wie z.B. in Dialysezentren und in der Neurochirurgie, für die bereits Studien geplant sind.
Die selektive intraarterielle digitale Subtraktionsangiografie (DSA) ist nach wie vor der Goldstandard zur Darstellung und Beurteilung extra- und intrakranieller Stenosen. Im Gegensatz zur extrakraniellen A carotis interna fehlt für die Stenosegradbestimmung der intrakranieller Arterien ein valides Messverfahren. Da Aussagen zur Prognose symptomatischer intrakranieller Stenosen und die Entscheidung über die weitere therapeutische Vorgehensweise jedoch weitgehend vom Stenosegrad abhängen, ist ein standardisiertes Verfahren zur Stenosegradbestimmung in Prozent für die intrakraniellen Gefäße von entscheidender Bedeutung. Die im Rahmen der WASID-Studie vorgeschlagene Methode eines lokalen, proximal der Stenose gelegenen Referenzdurchmessers wurde außerhalb dieser Studie noch kaum weitergehend untersucht. In der vorliegenden Studie wurden die Angiogramme von 102 Patienten mit intrakraniellen Stenosen nach den WASID-Kriterien von 3 Observern jeweils zweimal vermessen und bezüglich der inter- und intraobserver Korrelation untersucht. Die im Rahmen der WASID-Studie vorgeschlagene und für die vorliegende Studie übernommene Methode der Stenosegradberechnung erfolgte anhand eines zunächst proximal der Stenose zu wählenden Referenzdurchmessers. Eine modifizierte Stenosegradberechnung mittels Mittelwertbildung aus proximalem und distalem Referenzdiameter wurde der WASID-Methode gegenübergestellt. Die interobserver Korrelation der WASID-Methode lag bei 89,9% für den ersten Messdurchgang und bei 90,1% für den zweiten Messdurchgang. Die Stenosegradbestimmung nach der Mittelwert-Methode ergab eine geringfügig höhere interobserver Korrelation von 90,9% bzw. 93,2%, allerdings ohne statistische Signifikanz. Die Unterschätzung (im Vergleich zur WASID-Methode) des Stenosegrades durch die Mittelwert-Methode ist jedoch signifikant (am ehesten messverfahrensbedingt). Die vergleichende Untersuchung von proximalem und distalem Referenzdiameter im Rahmen der Mittelwert-Messung ergab bezüglich der Korrelation keinen statistisch signifikanten Vorteil für einen der beiden Messpunkte. Die Analyse der intraobserver Korrelation zeigte ebenfalls keinen eindeutigen Vorteil für die Mittelwert-Methode, allenfalls lässt sich hier ein positiver Trend ausmachen. Somit liegen letztlich die inter- und auch die intraobserver Korrelationen beider Messverfaheren auf vergleichbar hohem Niveau, ein statistisch klarer Vorteil für eine der beiden Vorgehensweisen lässt sich nicht ableiten. Aufgrund der einfacheren Handhabung der WASID-Methode empfiehlt sich dieses Verfahren dennoch für den klinischen Alltag.
Background/Objectives: Agility and cognitive abilities are typically assessed separately by different motor and cognitive tests. While many agility tests lack a reactive decision-making component, cognitive assessments are still mainly based on computer-based or paper-pencil tests with low ecological validity. This study is the first to validate the novel SKILLCOURT technology as an integrated assessment tool for agility and cognitive-motor performance.
Methods: Thirty-two healthy adults performed agility (Star Run), reactive agility (Random Star Run) and cognitive-motor (executive function test, 1-back decision making) performance assessments on the SKILLCOURT. Cognitive-motor tests included lower limb responses in a standing position to increase the ecological validity when compared to computer-based tests. Test results were compared to established motor and agility tests (countermovement jump, 10 m linear sprint, T-agility tests) as well as computer-based cognitive assessments (choice-reaction, Go-NoGo, task switching, memory span). Correlation and multiple regression analyses quantified the relation between SKILLCOURT performance and motor and cognitive outcomes.
Results: Star Run and Random Star Run tests were best predicted by linear sprint (r = 0.68, p < 0.001) and T-agility performance (r = 0.77, p < 0.001), respectively. The executive function test performance was well explained by computer-based assessments on choice reaction speed and cognitive flexibility (r = 0.64, p < 0.001). The 1-back test on the SKILLCOURT revealed moderate but significant correlations with the computer-based assessments (r = 0.47, p = 0.007).
Conclusion: The results support the validity of the SKILLCOURT technology for agility and cognitive assessments in more ecologically valid cognitive-motor tasks. This technology provides a promising alternative to existing performance assessment tools.
Das Gehirn als Hypoxie-empfindlichstes Organ des menschlichen Körpers ist auf eine konstante Blutversorgung angewiesen. Zur Gewährleistung der Unabhängigkeit von systemischen hypo- bzw. hypertensiven Blutdruckschwankungen, existieren verschiedene Regulationsmechanismen, welche den cerebralen Blutfluss (CBF) in einem weiten Bereich (arterieller Mitteldruck zwischen 60 - 150mmHg) konstant halten. Zu diesen Mechanismen gehören die Blutdruck-adaptierte Autoregulation, die metabolische Vasoreaktivität sowie die neurovasculäre Kopplung. Außerhalb des physiologischen Autoregulationsbereichs folgt der CBF druckpassiv den Schwankungen des systemischen Blutdrucks. Ischämische Hypoperfusion oder Gefäßwand-schädigende Blutdruckspitzen können die Folge sein. Zusätzlich führen verschiedene cerebrovasculäre Erkrankungen zu einer Einschränkung der cAR. Zu diesen gehören die cerebrale Mikroangiopathie, Stenosen bzw. Verschlüsse der A. carotis interna (ACI), der Schlaganfall sowie das Schädel-Hirn-Trauma (SHT). Eine valide Untersuchungsmethode zur Messung der cAR mit geringem apparativen Aufwand und fehlender Strahlenbelastung ist die transcranielle Doppler-Sonographie (TCD). Allerdings stellt eine Verfälschung der Messergebnisse durch verstärkte Streuung des Ultraschallsignals an der Schädelkalotte (schlechtes Schallfenster [sSF]) eine relevante Fehlerquelle dar. Mit steigendem Lebensalter finden sich in bis zu 30% der Bevölkerungen insuffiziente Schallbedingungen. Ziel der vorliegenden Studie war es daher festzustellen, ob die Dauerinfusion eines Ultraschallkontrastmittels (Levovist®) bei der TCD-Autoregulationsmessung an Probanden mit künstlich verschlechtertem Schallfenster eine valide Messung verschiedener Autoregulationswerte ermöglicht. 45 unselektierte Personen mit gutem Schallfenster aus dem Patientengut der Neurologie der J.W.G.-Universität Frankfurt am Main wurden in die Studie eingeschlossen. Die Doppler-Signalqualität, die Blutflussgeschwindigkeiten, der Pulsatilitätsindex nach Gosling und King (PI) sowie die Autoregulationsparameter Phasendifferenz (PD) und Kreuzkorrelations-Koeffizient (Mx) wurden anhand der über das transtemporale Schallfenster abgeleiteten A. cerebri media (ACM) beidseits bestimmt. Zur Imitation des sSF wurde eine Lage Aluminiumfolie ohne Veränderung der Sondenposition zwischen Ultraschallsonde und Haut eingebracht. Die Messungen erfolgten nach einem standardisierten Protokoll in drei Durchgängen (native Messbedingungen, künstliche Schallfensterverschlechterung, Infusion des Ultraschallkontrastmittels (KM) Levovist® [300 mg/min] bei bestehendem sSF) in einem Zeitraum von je 15 Minuten in Ruhe und je 3 Minuten während metronomischer Atmung. Mit künstlicher Verschlechterung des Schallfensters zeigte sich eine signifikante Verfälschung aller erhobenen Messparameter. Die mittlere Flussgeschwindigkeit in der ACM und die mittlere Energie des Doppler-Spektrums verringerten sich um 40%, bzw. um 22%, wohingegen sich der PI verdreifachte. Ebenso veränderten sich die Autoregulationsparameter mit einer Abnahme der PD um durchschnittlich 8 - 10° und des Kreuzkorrelations-Koeffizienten Mx von 0.308 +/- 0.170 auf 0.254 +/- 0.162. Die Verringerung der PD täuscht eine pathologische Einschränkung der cAR vor. Diese Einschränkung entspricht z.B. der einer 75 - 99%-igen Stenose der ACI und kann darüber hinaus auch bei einer Reihe weiterer cerebrovasculärer Erkrankungen (z.B. lakunäre Infarkte bei cerebraler Mikroangiopathie, Subarachnoidalblutung [SAB], maligne Hypertonie) beobachtet werden. Die Abnahme des Kreuzkorrelations-Koeffizienten Mx täuscht im Gegensatz dazu eine scheinbar intakte cAR (Mx < 0.3) vor. Bei Patienten mit schwerem Schädel-Hirn-Trauma (SHT), SAB oder Stenose der ACI besteht in diesem Fall die Gefahr, eine optimale Prävention von Sekundärschäden nicht gewährleisten zu können, da frühe Hinweise auf eine Schädigung des Gehirns, ausgelöst durch verminderte cerebrale Autoregulation, unter Umständen fehlen. Die Infusion des Ultraschallkontrastmittels Levovist® konnte diese Effekte der künstlichen Verschlechterung des Schallfensters auf die Autoregulationsparameter über den gesamten Messzeitraum fast vollständig ausgleichen (PD nativ vs. KM 38.2 +/- 10.0 vs. 37.2 +/- 12.3 und Mx nativ vs. KM 0.308 +/- 0.170 vs. 0.323 +/-0.163). Auch die übrigen erfassten Messparameter näherten sich durchweg den Werten unter nativen Messbedingungen an. Die Verwendung eines Ultraschallkontrastmittels bei der transcraniellen Doppler-Sonographie erhöht also die Validität der Messung der cerebralen Autoregulation bei Patienten mit schlechtem Schallfenster. Eine klinische Relevanz entsteht im besonderen, da sowohl ein schlechtes Schallfenster, als auch cerebrovasculäre Erkrankungen, welche im Zusammenhang mit einer verminderten cerebralen Autoregulation stehen, bevorzugt bei älteren Menschen anzutreffen sind.
Whereas the lack of biomarkers in penile cancer (PeCa) impedes the development of efficacious treatment protocols, preliminary evidence suggests that c-MET and associated signaling elements may be dysregulated in this disorder. In the following study, we investigated whether c-MET and associated key molecular elements may have prognostic and therapeutic utility in PeCa. Formalin-fixed, paraffin-embedded tumor tissue from therapy-naïve patients with invasive PeCa was used for tissue microarray (TMA) analysis. Immunohistochemical staining was performed to determine the expression of the proteins c-MET, PPARg, β-catenin, snail, survivin, and n-MYC. In total, 94 PeCa patients with available tumor tissue were included. The median age was 64.9 years. High-grade tumors were present in 23.4%, and high-risk HPV was detected in 25.5%. The median follow-up was 32.5 months. High expression of snail was associated with HPV-positive tumors. Expression of β-catenin was inversely associated with grading. In both univariate COX regression analysis and the log-rank test, an increased expression of PPARg and c-MET was predictive of inferior disease-specific survival (DSS). Moreover, in multivariate analysis, a higher expression of c-MET was independently associated with worse DSS. Blocking c-MET with cabozantinib and tivantinib induced a significant decrease in viability in the primary PeCa cell line UKF-PeC3 isolated from the tumor tissue as well as in cisplatin- and osimertinib-resistant sublines. Strikingly, a higher sensitivity to tivantinib could be detected in the latter, pointing to the promising option of utilizing this agent in the second-line treatment setting.
